OPMV#1 Stated she was making a right turn onto Commonwealth Ave and the OPMV#2 started entering her lane with no signal and struck the left side of her vehicle. OPMV#2 Was traveling Southbound on Lexington St entering the right turn only lane when she struck the OPMV#1. OPMV#2 Stated she was entering the turn lane to make a right turn onto Commonwealth Ave and didn't notice there was a car already in the lane. OPMV#2 Stated she felt distracted due to the heavy rain and lights at the intersection.

On Saturday March 9, 2019, at approximately 0230 hours, I, Officer Brandon, responded to the Weston/Newton line on Commonwealth Ave to assist Weston PD with field sobriety. On arrival Weston PD had concluded field sobriety and determined that the party, Glenn Matot, was not under the influence. Weston PD then explained that they had been contacted by AAA stating that a single vehicle accident occurred in Weston's jurisdiction. After further review, Weston PD and I determined that the accident was in fact in Newton. I then spoke with the operator of MV1, Glenn Matot, who stated that he was on his way home from work traveling westbound on Commonwealth ave in front of the Charles River Canoe and Kayak when he became distracted by a text message. Mr. Matot further explained that while texting, his car drifted to the right causing him to drive up on the snow bank and up onto the guard rail before then falling off back into the road. MV1 sustained major damage to both the passenger side and undercarriage. Mr. Matot did not sustain any injuries and denied any medical attention. AAA arrived on scene and removed the vehicle from the road and Mr. Matot took an Uber home. Two pictures were taken of the damaged City of Newton guard rail and submitted to IT to be attached to the report.

On 4/14/2019 at approx 0923Hrs while assigned to 497 I responded to the area of Commonwealth Av and Ash St for a report of a three car crash. upon arrival I observed three cars off to the side of the road. i spoke with the operator of ma Reg T6T, Robert MURPHY who stated he had stopped for the red light at Commonwealth and Ash St when the operator of TN Reg 2N37P1, Jennifer BURNELL failed to stop striking the operator of Ma Reg 8CL834 Evan GUIDRY who in turn than ran into the back of MURPHY. I spoke with BURNELL who stated she was lost and had been looking down onto her phone looking for a map when she ran into the back of GUIDRY. BURNELL cited for violation MGL90/13 distracted Operation , GUIDRY transported to NWH by Cataldo Ambulance for neck and head pain. GUIDRY and BURN ELLS vehicles towed by Todys. NFD responded and cleared roadway or debris and fluids.

On 04/27/2019, while assigned to N494, I, Officer Conary, responded to Commonwealth Ave at Lowell Ave for a MVA. Upon arrival, I observed two sedans in the middle of Commonwealth, facing opposite directions. I told both operators to pull to the side of the road. Operator of MV1 stated that he was traveling Southbound on Lowell Ave and took a left turn onto Commonwealth Ave going Eastbound. When he took the turn, he said he took it too sharp and got distracted with a lip balm in his car. He looked down because it fell on the side of the driver's side door. When he looked down he entered the Westbound lane of traffic and hit MV2. Operator of MV2 stated that she was traveling Westbound on Commonwealth Ave when she saw MV1 swerving into her lane. She attempted to avoid MV2 hitting her, but MV1 hit her head on. Both operators were offered and declined medical attention. Both vehicles were driven from scene. No further incident to report.

Operator of MV1(Ma reg 866MF0) stated he was traveling EB on Washington St when his vehicle was struck in the rear. Operator of MV1 stated he was stopped at the red light at the intersection of Jackson Rd. Operator of MV2(AMB1406) stated he was traveling EB on Washington St when he struck the rear of MV1. Operator of MV2 stated he observed a vehicle coming down from Lewis Terrace and was concerned they were not gonna stop at the intersection. This distraction caused MV2 to strike MV1 at a slow rate of speed. Both vehicles sustained minor damage and did not require a tow. The passenger and driver of MV1 were observed by the medics. The passenger was transported in the ambulance for undisclosed minor injury. The driver refused to sign the medical release. 5 Photos were taken of both vehicles showing the damage. I put the memory card in an envelope and left it for the I.T. Bureau.

Narrative:
On Thursday, August 8, 2019, while assigned to Traffic unit N525, I responded to the area of Centre Street and Mt. Ida Street for a report of a 3 car motor vehicle crash with entrapment. The weather at the time of the crash was clear and sunny. The road surface was dry. Centre Street and Mt. Ida Street are both public ways maintained by the City of Newton. On arrival, I was updated by Sgt. Chisholm that one of the vehicles (MV3) had a female operator that was entrapped as a result of the collision. She was freed from the vehicle by the Newton Fire Department and transported to Beth Israel Hospital. Another operator (MV2) was transported to Newton Wellesley Hospital with minor injuries. A third operator (MV1) remained on scene with no injuries. With the assistance of Officer Wade we conducted a survey of the crash scene. We observed a large debris field at the intersection of Centre Street and Mt. Ida Street with debris from all three vehicles. The point of impact was observed to be on Centre Street (N) before Mt. Ida Street. There were fresh scrapes in the pavement as a result of the colission. We also observed skid marks from MV1 measuring 57 feet 8 inches from the start of it's skid to the vehicles final rest MV1 crashed into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane of Centre Street and crash into MV3 as it was passing by. The impact from MV2's front end crashing into MV3's driver side resulted in the operator of MV3 becoming entrapped. MV2 and MV3 came to final rest on Wesley Street (W) at Centre Street (S). The driver door of MV3 was inaccessible. The collsion resulted in air bag deployment for MV2 and MV3. I observed moderate damage to MV1's front grill/bumper area. I observed substantial damage to the front and rear of MV2 and substantial damage to the front and driver side of MV3. Tody's responded and removed MV2 and MV3 from the roadway. Photos were taken of the scene and submitted to the IT Bureau. Officer Martin of the Watertown Police Department was requested by Sgt. Lee to respond to the scene and assist with a commercial truck inspection of MV1. Officer Martin contacted Trooper Grey of the Massachusetts State Police Truck Team who also responded to assist with the truck inspection. Sgt. Lee remained with Officer Martin and Trooper Grey during the inspection. MV1 was relocated to Richardson Street for the inspection. Trooper Grey determined the truck was in compliance with all required paperwork and it passed it's mechanical inspection. MV1 was removed from the roadway by a state tow. I spoke with the operator of MV1, William Tisbert (S24792655). Tisbert stated he was operating a 2006 KW W900 Semi Truck (NH SEMI: AP2638) Northbound on Centre Street. Tisbert stated as he was approaching Mt. Ida Street, a vehicle came out of no where and he hit it. Tisbert stated there was no one in front of him and then a vehicle came off of a side street (Mt. Ida Street) and attempted to turn left in front of him to return to the side street. Tisbert stated he could not avoid hitting the vehicle. Tisbert crashed into the rear of MV2. MV2 then crossed into the opposite lane and crashed into another vehicle. Tisbert remained on scene and reported no injuries. Tisbert stated he was wearing his seatbelt at the time of the accident and was not distracted. Tisbert later admitted to crashing into the rear of MV2. I spoke with the operator of MV2, Janet Vanbuskirk (S59829146), at Newton Wellesley Hospital. Vanbuskirk stated while operating her 2004 Toyota Highlander (MA: RS 529Y), she was stopped on Centre Street (N) at Mt. Ida Street. Vanbuskirk had her left directional on and was waiting for traffic in the Southbound lane to clear to take a left turn onto Mt. Ida Street. Vanbuskirk stated MV1 crashed into the rear of her vehicle. As a result of the impact, Vanbuskirk's vehicle traveled into the opposite lane and crashed into the driver side of MV3. Vanbuskirk reported minor injuries to her neck, left arm, and right hand. I spoke with the operator of MV3, Madeleine Karolian (S23979811), at Beth Israel Deaconess Medical Center in Boston. Karolian stated she was operating her 2016 Toyota Prius (MA: EV5746) Southbound on Centre Street. Karolian stated MV1 crashed into MV2, and MV2 crashed into her vehicle. Karolian stated she did not have time to avoid the collision. Karolian stated she had minor injuries to her neck and chest. Karolian's vehicle had a dash cam recording at the time of the accident. I viewed the footage with her through an app on a cell phone. The footage shows MV3 traveling Southbound on Centre Street on August 8, 2019 at 1308 hours. Ahead of MV3 in the Northbound lane, MV2 is seen stopped on Centre Street before Mt. Ida Street with it's left directional activated. I observed MV1 travel Northbound on Centre Street and crash into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane and crashed into the driver side of MV3. The footage then ends. A copy of the footage was emailed to my work email. The footage was then forwarded to IT Director Steve Smith to be added to the media for this report. As a result of my investigation, the operator of MV1, William Tisbert, will be cited for Newton City Ordinance Chapter 19, Section 71 (Following too closely). Massachusetts Uniform Citation T1270942 was completed and mailed to Tisbert's residence.

MV1 was parked in front of #96 McCarthy Rd. MV2 is a City of Newton Forestry truck that was occupied and parked behind MV1 waiting to move up to a work site. MV2 began moving forward towards the site and struck MV1 from behind. Operator states that he was distracted by repositioning his seat and forgot that MV1 was parked in front him. Narrative:
On Saturday, September 28, 2019, I responded to the intersection of Crafts Street and Albemarle Road, Newton with Sgt. Lee for a report of a motor vehicle crash involving a motorcycle with serious injury to it's operator. The weather at the time of the crash was clear and sunny. The road surface was dry. Crafts Street and Albemarle Road are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jussara Pontes (S92930243). Pontes stated she was operating a 2017 Toyota Sienna (MA: 4GX777) at the time of the crash. Pontes stated she was traveling North on Albemarle Road and came to a stop at the stop sign located at the Crafts Street intersection. Pontes stated she looked both ways and began to inch out into the roadway. Pontes stated she stopped the vehicle again and was assisting a child sitting in the 2nd row of the vehicle. Pontes stated she then turned her attention back to the roadway. Pontes stated she looked to her right and left again and did not see any vehicles in either direction. Pontes stated she proceeded to cross the intersection to continue North on Albemarle Road. Pontes stated the front bumper area of MV1 crashed into the left side of a passing motorcycle. Pontes stated the motorcycle and it's operator fell to the roadway and slid across the street to the side of the road. Pontes stated she pulled her vehicle over onto Albemarle Road. Pontes stated she did not see the motorcycle prior to the crash and was not distracted. The operator of the motorcycle (MV2), Irene Pistoftzian (S06099449) was transported to Brigham and Women's Hospital with a serious left leg injury prior to my arrival. I observed a red 1990 Kawasaki ZR550 (MA MCN: 1V1063) resting on it's right side on the NE sidewalk area of Albemarle Road at Crafts Street with it's front wheel/handlebar facing Crafts Street. The motorcycle had moderate damage to it's left side from the impact of MV1 crashing into it. The right side of the motorcycle had damage from sliding on the roadway and the crashing into a fence on the sidewalk on Crafts Street (W) at Albemarle Road. There was also damamge to the front headlight/right handlebar area. The motorcycle and Toyota Sienna were towed from the scene by Tody's towing at Sgt. Lee's request. Pictures were taken of the scene and submitted to the IT Bureau. I spoke with Pistoftzian at Brigham and Women's Hospital in Boston on Monday, September 30, 2019. Pistoftzian stated she was operating her motorcycle on Crafts Street (W). Pistoftzian stated as she approached the intersection of Crafts Street and Albemarle Road, she observed a mini van to her left out of her peripheral. Pistoftzian stated she remembers being hit on her left side and then laying on the roadway looking up at the sky on the sidewalk with her head facing Albemarle Field. Pistoftzian stated as a result of the crash she has a significant injury to her left leg and also has an injury to her left wrist/arm. Pistoftzian stated she was wearing a helmet at the time of the crash. I spoke with Kymberley Mitchell via telephone who stated she witnessed the crash. Mitchell stated she was traveling behind the motorcycle involved in the crash on Craft Street (W) at North Street. Mitchell stated they were stopped at a red light at this time. Mitchell stated there were no vehicles in front of the motorcycle at the light. Mitchell stated the light turned green and they continued on Crafts Street at a low rate of speed. Mitchell stated she could see ahead at the intersection of Albemarle Street and Crafts Street, traffic was stopped on Crafts Street (E) letting traffic exit Albemarle Road (N). Mitchell stated vehicles were taking left and right turns out of Albemarle Road before the motorcycle reached the intersection. Mitchell stated a mini van was crossing past the double yellow line of Crafts Street to continue straight when it crashed into the left side of the motorcycle. I spoke with Cheryl Wermer via telephone who stated she witnessed the crash. Wermer stated she was traveling on Crafts Street (E) towards Albemarle Road. Wermer stated traffic ahead of her was stopped to allow vehicles to turn off of Albemarle Road. Wermer stated she observed a motorcycle traveling towards her on Crafts Street (W). Wermer stated she then saw a vehicle travel straight from Albemarle Road across Crafts Street and crash into the motorcycle. Wermer stated the operator of the motorcycle flew off the bike. Wermer stated the car then came to a stop and the driver got out and stated "Oh my god! I didn't see you!" Wermer stated both vehicles were traveling at a low rate of speed prior to the crash. I spoke with Bill Frabizio via telephone who stated he witnessed the crash. Frabizio stated he was behind the mini van (MV1) on Albemarle Road prior to the crash. Frabizio stated the vehicle in front of him began to cross the intersection as he was monitoring the traffic on to take a right turn onto Crafts Street. Frabizio stated he heard a loud "Boom" and saw a motorcycle slide across the street with its operator. Frabizio stated the motorcycle crashed into a fence post and the operator came off the motorcycle. Frabizio stated he pulled over and assisted operator of the motorcycle. After concluding my investigation, the operator of MV1, Jussara Pontes, was mailed Massachusetts Uniform Citation T2016424 for Newton City Ordinance Chapter 19, Section 75 (Failure To Use Care).

On October 12, 2019 at approximately 2347 hours, I responded along with multiple NPD units to a report of a MV into a pole at the intersection of Washington St and Commonwealth Ave. Upon arrival, I met with Officer Boyle who directed me to a 2007 Jeep Wrangler(MA Reg:7KK957) that was popped up onto the curb and through a traffic light pole in the intersection on the Westbound side of commonwealth Ave. I was then directed over to the vehicles operator, Sebastian Pike. Sebastian was outside of the vehicle walking around when I had arrived. Sebastian was directed to sit down for now due to airbag deployment from the accident. Sebastian was not complaining of any pain and did not need any medical attention. I then asked Sebastian what happened tonight. Sebastian stated he had just left a friends house and was driving the Jeep Westbound on Commonwealth Ave. He was driving through the intersection with Washington St when he became distracted and looked away from the road. At this point, his Jeep had jumped the curb and crashed into the traffic light pole. The traffic light pole was knocked down and blocked the road Westbound on Commonwealth Ave. Cruisers were used to block the road until Daegle Electric had arrived on scene to clear away the debris. The Jeep was towed by Tody's and a towed motor vehicle form was filed. Due to Sebastian's failure to pay attention, Sebastian Pike was issued Massachusetts Uniform Citation #T2079375 for 90/24 Negligent Operation and 89/4a Marked Lanes Violation. A copy of the report was attached to the citation and submitted to the police prosecutors box as a Criminal Application. Sebastian's Father, Jeffrey Pike, had arrived on scene and drove his son home for the night. All aforementioned streets and Avenue's are public ways in the Commonwealth of Massachusetts.

MV #1 was stopped northbound in traffic on Centre St. by Alden Place. MV #2. was traveling northbound on Centre St. behind MV #1 when it collided into the rear bumper of MV #1. The operator of MV #2 stated she was distracted and was unable to stop MV #2 from colliding into MV #1. There was visible damage to MV #1's rear bumper while MV #2 had damage to its front bumper as well as the passenger side headlight. Both operators declined medical attention. Both vehicles were able to be driven away from the scene.

Narrative:
On Monday, November 4, 2019, while assigned to Traffic unit N525, I responded to the intersection of Walnut Street and Walnut Place, Newton for a report of a motor vehicle crash involving a pedestrian. The weather at the time of the accident was clear and sunny. The road surface was dry. I spoke with the injured pedestrian, Ernestine Gianelly, as Newton Medics were preparing her for transport to the hospital. Gianelly stated she was crossing Walnut Street in a marked crosswalk towards Washington Park after leaving the Newton Senior Center located at 345 Walnut Street, Newton. Gianelly stated a vehicle exited Washington Park and turned left onto Walnut Street directly at her. Gianelly stated she believes the driver did not see her. Gianelly stated the vehicle hit her while she was in the marked crosswalk on her left side. Gianelly stated she then fell to the roadway. Gianelly was taken to St. Elizabeth's hospital in Boston for injuries to her ribs, back, and sides. I spoke with the operator of MV1, Alex Cook-Williams (S13317988). Cook-Williams stated he was operating his 2009 Pontiac Vibe (MA: 222FV5) Westbound on Washington Park towards Walnut Street. Cook-Williams stated he came to stop at the intersection and observed no traffic in either direction of Walnut Street. Cook-Williams stated he made a left turn onto Walnut Street and did not see the crossing pedestrian until it was too late. Cook-Williams stated he slammed on his brakes and the pedestrian landed on his hood. Cook-Williams stated the pedestrian then fell to the roadway. Cook-Williams reported no injuries. Cook-Williams stated he did not know if the pedestrian was in the crosswalk or not at the time of the crash. I observed minor damage to the passenger side hood/bumper area. Cook-Williams stated he was not distracted at the time of the accident. After speaking with all parties involved, Cook-Williams was issued Massachusetts Uniform Citation T2016426 for Chapter 89, Section 11 (Fail to Yield, Pedestrian in Crosswalk). Walnut Street, Walnut Place, and Washington park are all public ways maintained by the City of Newton. Photos were taken and submitted to the IT Bureau.

Operator of vehicle one , Allan Wint stated on 01/27/2019 at 09:40 hours he was driving his employer's pick up truck south bound on Lexington St near number 227. Lexington St is a public way in the city of Newton. Wint is a driver for Auto Zone auto parts located at 140 Lexington St , Waltham, MA ( 781 894-0656 ) . Wint was driving a 2018 Chevrolet Colorado pick up tuck ( MA reg 8CA823 ) that Auto Zone leases from Lease Plan USA Ltd. Wint stated that he was headed to Webster St in Newton to pick up a delivery. Wint stated that as he approached the area of 277 Lexington St he believed that the traffic light that is at the marked crosswalk was green for south and north bound traffic. Wint stated that he did not see any pedestrians in the crosswalk, until he heard the sound of an object impacting into the front driver's side of his vehicle. Wint stated that he stopped his vehicle approximately ten yards past the crosswalk and got out. Wint stated it was then he realized that his vehicle had stuck a female and a dog. Wint stated that he was not operating any electronic devices at the time of the crash, and I observed none in his vehicle. Wint stated he was not injured in crash. Wint's vehicle had damage to the driver's side mirror and lower molding of the driver's side front door. Wint's vehicle did not require a tow. Witness Bessy Shanks stated that she was driving in her vehicle behind Wint's vehicle south bound on Lexington St. Shanks stated that Wint's vehicle was driving slow and swerving approximately two blocks prior to the crash. Shanks stated that she believed that Wint may of been distracted. Shanks stated that south and north bound traffic was stopped because that cross walk light was red and that a female and a dog were crossing from the east to west in the marked crosswalk in font 227 of Lexington St. Shanks stated that the front driver's side of Wint's vehicle srtuck the female and the dog as they were approximately halfway into the crosswalk. Shanks stated that the female and dog both fell to the ground after the crash. Witness Elizabeth Keegan stated that she was driving in her vehicle south bound on Lexington St two vehicle behind Wint's vehicle. Keegan stated that she was not sure what color the traffic light was at the time of the crash, but traffic was stopped both not and south bound traffic was stopped. Keegan also stated that Wint's vehicle " didn't even slow down ". Keegan stated that Wint's vehicle struck the female pedestrian and the dog as they were in the marked crosswalk in front of 277 Lexington St. Witness Henry Taylor stated that his vehicle was the first one stopped at the red crosswalk light at 227 Lexington St in the north bound lane. Taylor stated that north and south bound traffic was stopped and that Wint's vehicle did not stop and stuck a female pedestrian and a dog in the crosswalk. Witness Jacqueline Deos stated that her vehicle was stopped behind Taylor's north bound on Lexington St. Deos stated that north and south bound traffic was stopped for the red light . Deos stated that Wint's vehicle did not stop and that it struck a female pedestrian and a dog in the marked crosswalk. The pedestrian in the crosswalk was identified as Xiaoping Sun. Sun stated that she and her dog stopped on the south bound side of the marked crosswalk across from 227 Lexington St. Sun stated that she activated the crosswalk light and waited until the light was red for north and south bound traffic, and that the walk single was activated. Sun stated that she looked to her left and saw traffic north bound traffic was stopping. Sun stated that she turned to her right and entered the marked crosswalk. Sun stated that she and her dog had crossed over the north bound side of Lexington St and she then looked to her right and saw Wint's vehicle not stopping and " going very fast ". Sun stated that the front driver's side mirror of Wint's vehicle stuck her in the face . Sun stated that as she fell to the ground she was struck on the left leg by Wint's driver's side. Sun stated that she did not lose consciousness after she had fallen to the ground. Sun was transported via EMS to The Brigham and Women's Hospital for treatment for head and neck injuries and abrasions to her legs. Sun's dog died as a result of being struck by Wint's vehicle. Photo's of the crash site and of Wint's vehicle were taken and the disc was downloaded by the NPD's IT Bureau. Before leaving the crash site, I tested the crosswalk lights and they were in working order. Based upon my observations of the crash site, Wint's vehicle, and the statements made to me, I concluded that Wint was at fault for this crash. I issued Wint MA Citation T 2013958 and cited him for a violation of MGL 89/11 , failure to yield to a pedestrian in a crosswalk.

On 02/14/20 at approximately 14:45 I responded to a crash in the parking lot outside of #50 Winchester St- The Tile In Style store. Upon my arrival the Fire Department was already there and I was met by the owner of the building Bruno DiFazio. The vehicle that had struck the building had left the scene but he showed me security footage of the crash. The video showed a white S.U.V. (later determined to be Mass. Reg# 6GEL40 White 2017 Acura RDX owned and operated by Eleonora Revzin) pull slowly into a parking spot and then accelerate up and over the curb and into the building. The crash caused moderate damage to the brick facade and inside wall of the tile store. A short time later, Revzin drove back into the parking lot where I spoke with her. Revzin stated she didn't know why her car had accelerated into the building but she was having some personal issues and may have been distracted. She explained that she left the area because she was shaken up but then as she had time to clear her head, thought she should return to the store. There was minor damage to her vehicle and she stated she was not hurt and didn't want me to call the Medics for an evaluation. I issued Revzin Mass. Citation #T2013436 in hand for N.C.O. 19/75- Failure to use care in stopping and a verbal warning for leaving the scene. City of Newton Inspector Paul Nelson showed up and deemed the building to be structurally safe. N523 Officer Wilson arrived and took pictures of the scene.

On 03/11/2020, while assigned to N495, I, Officer Conary, responded to the intersection of Centre Street at Commonwealth Ave for a minor motor vehicle accident. Upon arrival, MV1 passenger was getting evaluated by the medics. I spoke to the Operator of MV1 and he stated he was traveling Northbound on Centre street and stopped at the red light at the intersection. He began to roll forward only a couple feet then he came to a complete stop again. At this time, there was a green left signal only for the left lane only. MV2 hit MV1 from the rear. Operator of MV1 said the other driver was distracted. Operator of MV2 traveling Northbound on Centre Street, behind MV1, came to a stop at intersection. She saw the green left signal only and proceeded forward hitting MV1. Operator of MV2 said that she wasn't paying complete attention. Both vehicles were able to be driven from scene. There was minor damage to MV1 rear bumper. There was minor damage to MV2 front bumper. MV1 passenger signed a patient refusal. No further incident to report.

Mv#1 was travelling on Kenwood Ave S/B, veered off the road to the right and struck a city light gas pole in front of #11 Kenwood. #1 operator stated she got distracted by admiring the blooming flowers on a tree across the street, taking her eyes off the road resulting in the collision with the pole. Pole broke in half near the middle of it. NFD was able to shut the gas off. National Grid was notified to respond. City was notified of the pole damage. On Sunday, 5/31/20 at 1652 hrs, I (N493) responded to the area of 432 Cherry St. for a single motor vehicle collision. Upon arrival, I observed MV#1 (MA Reg: 6WS227) had collided into utility pole ( 249/47) adjacent to 432 Cherry St. Damage was observed to MV#1's entire front end. The operator stated he was traveling northbound on Cherry St. when he became distracted, lost control of the vehicle and collided into the utility pole. The operator of MV#1 stated he was not injured and signed a patient refusel. Tody's Tow service was contacted to remove the vehicle from the roadway. On June 13, 2020 Officers responded to 129 Parker St for a report that a vehicle crashed into a parked car. Upon arrival we spoke with the operator of MV1 who was being evaluated by medics. The operator of MV1 declined further medical attention for leg and back pain. The operator of MV1 informed me that he was traveling southbound on Parker St. when he became distracted by his cell phone GPS falling on the floor. He went to retrieve the item and struck the rear of MV2 which was parked in front of 129 Parker St. The vehicle owner of MV2 was on scene and was advised of the situation. Due to the damage, both vehicles were towed from the cent by Tody's towing.

Based on observations and statements made, the following occurred. On 07/15/2020, while assigned to N494, I, Officer Conary, responded to the intersection of Centre Street at Church Street for a motor vehicle accident. Upon arrival, I spoke with Operator of MV1 who stated that he was driving Southbound on Centre Street when the car in front of him stopped suddenly at the green signal. At this time, MV1 hit MV2 rear. Operator of MV1 said that he believes she was distracted with a hand held device. Operator of MV2 explained to me that she at the intersection of Centre Street and Church Street traveling Southbound. She got the green signal and she let the car traveling Northbound take a left turn onto Church Street before she continued through the intersection. At this time, MV2 was hit by MV1 from the rear. She stated she was not using her phone. It should be noted that the signal traveling Northbound on Centre is a green left signal to turn onto Church Street. After the green arrow, cars yield to oncoming traffic to turn. Fire and Medics arrived on scene and all parties involved signed patient refusals. MV1 was towed by AAA. MV1 had minor damage to the front bumper. MV2 was able to be driven from scene with minor damage to the rear bumper. No further incident to report.

MV#1 was traveling southbound on Walnut St. when the operator of MV#1 reported she became distracted while retrieving an item from the passenger seat. This inattention caused MV#1 to collide into the rear of MV#2 which was parked (unoccupied) facing southbound on Walnut St. MV#1 sustained heavy damage to its front end, while MV#2 sustained heavy damage to its rear end. Both vehicles were towed from the scene by Tody's Towing Service. The operator of MV#1 declined medical attention.

On Tuesday, October 6, 2020, I received a call at approximately 2158 hours from Sgt. Devine of the Newton Police Traffic Bureau. Sgt. Devine stated there was a motor vehicle crash with serious injury that had just occurred at the intersection of Crafts Street and Waltham Street in Newton. Sgt. Devine stated that one of the parties involved had suffered life threatening injuries and was transported to Brigham and Women's Hospital. I responded to the scene and conducted an investigation of the crash with the assistance of Sgt. Devine and Captain Boudreau. Upon arrival it was reported to me by officers on scene that a party operating a motorized scooter on Crafts Street (E) had been struck by a 2007 Chrysler Town and Country (MA: 5EE316). The Chrysler was attempting to make a left turn from Crafts Street (W) onto Waltham Street (S). The operator of the motorized scooter, identified as Nadhir Tchikou (DOB: 11/23/1993), had suffered serious injuries to his head and left leg area and was transported to Brigham and Women's Hospital. It was reported to me that Officers Conary, Boyle, and Donovan were the first to arrive at the crash and provided life-saving care to Mr. Tchikou prior to being transported to Brigham and Women's Hospital. Officer Donovan responded to Brigham and Women's to keep us updated on Mr. Tchikou's status. The update provided to me while on scene was that Mr. Tchikou was in critical condition and in surgery. Due to the seriousness of the crash and potential fatal injury, Sgt. Devine contacted the Massachusetts State Police to assist in the investigation. Sgt. Mahoney and Trooper Paul responded as part of the Massachusetts State Police Crash Reconstruction team and conducted a supplemental investigation of the crash (MSP report 2020-CAR-000285). The location where the crash took place is Crafts Street and Waltham Street. Crafts Street and Waltham Street are public ways maintained by the City of Newton. Crafts Street and Waltham Street are municipal roadways that support two way travel for motor vehicles and bicycles with an unprotected median in the middle of both roadways to separate the direction of travel. This intersection was recently re-paved within the last month and has newly painted street markings including crosswalks, dedicated bicycle lanes, and stops lines at the intersections. Crafts Street at this intersection features two travels lanes. I observed the road surface to be dry at the time of my arrival on scene. The north lane in the westbound direction of Crafts Street permits travel straight ahead and also permits motorists/bicyclist to take a right turn onto Harding Street. The south lane in the westbound direction of Crafts Street is a dedicated left turn only lane that coincides with a left turn only arrow on the traffic signal in the middle of the intersection. The opposite side of Crafts Street permits traffic traveling in the eastbound lane to travel straight or take a left turn onto Harding Street from is a single lane travel lane. The area is well lit featuring street lights bright enough to illuminate the intersection for motorist traveling through the area. The weather at the time of the crash appeared to be clear. In the intersection where the crash took place, I observed a large debris field comprised of parts from a 2020 Tao Tao motorized scooter (49CC) and a 2007 Chrysler Town and Country (MA: 5EE316). I observed the motorized scooter (VIN: L9NTCBAG1L1010287) resting on its side facing North in the northbound lane of Crafts Street in the intersection. The motorized scooter has significant damage to its front, right, and left side and was inoperable. The area in the immediate vicinity of the motorized scooter was saturated with gasoline. The rear lights and dash of the motorized scooter were illuminated while I was observing the crash scene. The headlights were not intact due to the impact from the crash and I was unable to determine if they were functioning. The motorized scooter was removed from the roadway by Tody's Towing. The intersection where the crash took place had numerous first responders present immediately after the crash rendering aid to Mr. Tchikou prior to his transport to the hospital. It was reported by officers on scene that some of the items and debris in the intersection had been moved from their initial place of rest after the crash took place. A black MMG helmet was observed resting on the ground outside the debris field from the crash before the westbound left turn only lane of Crafts Street. It is unknown if it was moved from its initial place of rest by a first responder. I observed the strap on the helmet that would be used to secure it to the operator to be in good operable condition. I did not observe it to be defected and the securing feature of the strap was in working order. I did not observe any blood, hair, or other apparent physical evidence inside the helmet that would be consistent with the head injury reported to the operator of the motorized scooter. I observed what appeared to be fresh markings on the front mouth/nose area of the helmet. I observed a white Iphone with a red phone case lying on the roadway a short distance away from the impact area. The Iphone was on and displaying travel directions from the Iphone application Waze. The Iphone and the helmet were seized pending the status of the operator of MV2. A property/evidence form was completed and the Iphone and helmet were secured in property locker number 9. I spoke with the operator of MV1, William Moran (S34946215). Moran stated he was operating his white 2007 Chrysler Town and Country (MA: 5EE316) on Crafts Street (N) as he approached Waltham Street. Moran stated he had a green light and activated his left blinker as he approached the intersection with the intention of taking a left turn onto Waltham Street. Moran stated as he proceeded to take his left turn he saw "orange" and was unable to avoid hitting the object in front of him. Moran stated the object came out of nowhere and he swerved to the left in an attempt to avoid the crash. Moran stated after the collision he accelerated away from the impact area and came to a controlled stop on Waltham Street (S) just past the intersection. Moran stated he exited his vehicle and looked back at the intersection he had just travelled through. Moran stated as he began walking towards the intersection he observed a party laying on the roadway in the intersection. Moran stated the party was not wearing a helmet at this time. Moran also added that he believes the moped involved in the crash did not have its headlights on prior to impact. Moran stayed on scene and cooperated fully with officers during the investigation. A query of Moran's license and registration status through Newton Police Dispatched showed they are both active. I asked Moran if he was using a cell phone, GPS, or any other type of hand held device prior to the crash. Moran stated he was not. I asked Moran if he was distracted by anything either inside the vehicle or something on the roadway prior to the crash. Moran stated he was not. I asked Moran if he was currently impaired in any way that would impede his ability to operate a vehicle safely. Moran stated he was not. I asked Moran if he takes any medications or has a medical condition that would impede his ability to operate a vehicle safely. Moran stated he does not. Moran reported no injuries as a result of the crash. I observed Moran's vehicle at its controlled rest location. The vehicle was parked on Waltham Street (S) in the southbound lane just past the intersection of Crafts Street. The 2007 Chrysler Town and Country (MA: 5EE316) had substantial damage to the front passenger side bumper, hood, fender, windshield, and passenger door area. There was physical evidence of what appeared to be hair, blood, and tissue presumed to be from the operator of MV2 present in the cracked windshield, and on the passenger side door/mirror area. I observed a blue Commonwealth of Massachusetts "R" inspection sticker with a date of 05/28/2020 affixed to the windshield. Moran stated the rejection sticker was a result of an emissions failure. The inspection sticker expires in 2021. I asked Moran if there were any other mechanical issues with the vehicle. Moran stated there were no issues to his knowledge. I observed no objects present inside the vehicle during my investigation that would have potentially impeded the operators ability to see clearly through the windshield. I observed glass debris from the windshield inside the passenger and driver area of the vehicle. There was no air bag deployment inside the vehicle. Subsequent to my investigation, the vehicle was seized pending the status of the operator of MV2. The vehicle was towed from the scene by Tody's towing and secured in the Newton Police Department garage. The vehicle was escorted to the garage by Officer Donovan. A towed motor vehicle form was completed by Officer Donovan and the key for the vehicle was secured in property locker number 9. A property/evidence form was completed. Subsequent to my investigation Moran's black Alcatel cell phone was seized pending the status of the operator of MV2. A property/evidence form was completed and the phone was secured in property locker number 9. Officer Conary stated to me that prior to my arrival on scene she interviewed a witness to the crash and provided me with the following information. Joshua Hamel was operating his vehicle (W) in the right lane of Crafts Street towards Waltham Street. Hamel stated that MV1 (MA: 5EE136) was in the left turn lane on Crafts Street (W) towards Waltham. Hamel stated the scooter was headed eastbound on Crafts Street from Waltham. Hamel stated MV1 began to take a left turn and there was a head on collision with the scooter. Hamel stated he believed the traffic light at the time of the crash was green. On Friday, October 9, 2020, I spoke with Hamel via telephone. Hamel corroberated the statement he gave to Officer Conary. I asked Hamel if he believed the headlights on the motorized scooter were activated at the time of the crash. Hamel stated he believes the headlights on the motorized scooter were on as it approached the intersection but was not postive. On Wednesday, October 7, 2020, I spoke with Mr. Tchikou in his ICU room at Brigham and Women's Hospital. Tchikou's wife, Yami Carrajza, was present in the room while I spoke with him. Tchikou stated he had a significant head injury, injuries to his nasal cavity, his back, and a significant injury to his left leg. I asked Tchikou about his recollection of the crash. Tchikou stated he had just left his friend's home on Hazelhurt Avenue in Newton and was traveling home to Brighton to bring his wife dinner. Tchikou stated he was traveling Eastbound on Crafts Street as he approached the intersection of Crafts Street and Waltham Street. Tchikou stated he had a green light and as he entered the intersection a white vehicle took a turn from the opposite side of the road in front of him. Tchikou stated he does not remember anything after that. I asked Tchikou if he was wearing a helmet at the time of the crash. Tchikou stated he was. I explained to Tchikou that I recovered a white Iphone with a red case at the scene of the crash. Tchikou identified the Iphone as his. I explained that the phone was displaying travel directions when I found it and I asked Tchikou if he was distracted by his phone prior to the crash. Tchikou stated he was not distracted by the phone and the phone was in a holder on the motorized scooter. I asked Tchikou if he remembers if his headlights were activated on his motorized scooter at the time of the crash. Tchikou stated they were on. I asked Tchikou if he could produce paperwork for the motorized scooter to prove he was the owner. Tchikou stated he had recently purchased the scooter and the paperwork for it was at his residence. Tchkou's wife e-mailed me a picture of the Certificate of Origin for the motorized scooter. I asked Tchikou if he could produce a license or permit that would allow him to operate the motorized scooter. Tchikou stated he did not have a Massachusetts license or permit, but had an Algerian license at this residence. I was able to observe a picture of the license on his wife's cell phone. The written text on the license was in Arabic. Tchikou did not have a translation of the license in English and did not possess an International Driver's Permit. As a result of my investigation, Mr. William Moran while operating MV1 (MA: 5EE316) entered the travel lane of MV2 (Tchikou) as he was making a left turn onto Waltham Street from Crafts Street. MV2 had the right of way and MV1 failed to yield to MV2's right of way causing the collision to take place. Moran was advised of my investigation and issued Massachusetts Uniform Citation T0115914 via mail by request for Chapter 90, Section 8 (Fail To Yield To The Right Of Way At An Intersection.) Mr. Nadhir Tchikou operated his motor scooter at the time of the crash without a valid license/permit. Tchikou is also in violation of operation of a motor scooter after dark (prohibited). The crash occurred at 2147 hours. Tchikou will be issued Massachusetts Uniform Citation T0115915 in hand at Brigham and Women's Hospital for Chapter 90, Section 1E (Unlicensed Operation Of A Motor Scooter) and Chapter 90, Section 1E (Operation Of A Motor Scooter After Sunset - Hours Restriction.)

On my arrival I located the two involved vehicles on the other side of the intersection on Lewis St. Vehicle #1 a grey 2016 Gray Mazda SUV, Ma reg 7vb971 operator identified as Katherine Anna Erler. She reported that she was stopped S/B on Lewis Terrace @ Newtonville Ave when vehicle #2 drove into the front of her vehicle as he turned left from Newtonville Ave. Vehicle #2 was a Red Grand Jeep Cherokee, Ma reg 1HKT75 operated by Paul Damore. He reported being on Newtonville Ave E/B attempting to turn left(N/B) onto Lewis Terrace when he got distracted by another vehicle coming from Lewis St and drove into vehicle #1. All streets mentioned in this crash are public ways maintained by the city. There were no injuries in this crash and neither vehicle needed to be towed.

On, 11/19/2020 at approximately 1700 hrs, Newton Police and Fire were dispatched to Commonwealth Ave. and Prince St. for a reported head on motor vehicle collision with injuries. Upon arrival, I observed MV#1 ( 1WDR15) facing eastbound in the westbound travel lane. MV#2 (986KW1) was also facing eastbound, but in the eastbound travel lane. Both vehicles had significant front end damage, consistent with a head on motor vehicle collision. The operator of MV#1 was sitting on the grassy median upon my arrival. She stated she was traveling eastbound on Commonwealth Ave. when she became distracted by the radio, crossed over the double yellow line into the westbound lane of Commonwealth Ave. and collided with MV #2. The operator of MV#2 was entrapped in her vehicle, but was able to be extracted from the vehicle by Newton Fire personnel. Operator #2 was able to inform me that she was traveling westbound on Commonwealth Ave. She noticed MV#1 cross over the double yellow line at a high rate of speed, and collide into her vehicle. The operator of MV#2 was conscious and alert and transported to Tufts Medical Center Boston for an evaluation. The operator of MV#1 declined medical attention and was given a Massachusetts Uniform Citation (T2014484) for violating MGL. Ch. 89. Sec. 4(a) Marked Lanes Violation. Both vehicles were towed away from the scene by Tody's Towing Service. Photographs of this collision were taken on the (N492) camera and will be attached to this report.

On 1/21/21 I was working N493 when I responded to the area 450 Lexington St for a report of damage to city property from an MVA. Upon arrival I observed a parking sign that had been broken in half, Parking Meters that had been ripped from the ground and completely smashed, and 1 MA reg plate 8AH567. The plate came back to 398 Wolcott St. where I spoke to Dylan Crook. Crook stated he had been driving MA reg 8ah567 when he crashed and then went home. Crook stated he was distracted and reaching down to get something when he went off the road. On February 1st, 2021 at approximately 07:53 hours while assigned to N491 I responded to MV crash in front of #41 Bridge St. On my arrival I observed a large white box style truck crashed into a gray SUV both vehicles with extensive damage blocking the street completely. I first identified the operator of the box truck, Maurizio Didonna. The truck belonged to Russo's & Sons out of Pleasant St Watertown Ma. Mr Didonna reported driving S/B on Bridge St when he temporarily got distracted before crashing into a parked vehicle in front of #41 Bridge St. The owner of the parked vehicle was not on scene but a male construction worker who was building a new house at #41 Bridge St identified the vehicles owner as Jose Ramirez. He was able to make contact with him and informed him about the accident. Both vehicles were towed from the scene. One by Todys. Russo's & Sons truck was towed by Pro Tech out of Watertown. Mr Didonna had a minor injury but refused any medical treatment. Note:The parked SUV had a lot of construction equipment in the rear but was removed by the construction workers from the job site at #41 Bridge St. The parked car was affiliated with the construction site. The vehicles owner was notified to where his vehicle was being towed to. .

On 2/12/2021, at 2315 hrs, Newton Police were dispatched to the area of Watertown St. and Brookside Ave. for a single motor vehicle collision. Upon arrival, MV#1 (Red, Jeep Grand Cherokee) was observed parked, on the side of the road facing eastbound in front of 687 Watertown St. MV#1 was observed to have front end damage and two flat tires (driver side). There were no reported injuries to the operator of MV#1 or its passenger. MV#1 was deemed mechanically sound and left the scene after being provided with the necessary information pretraining to this accident. MV#2 was parked next to MV#3 and was unoccupied when it was struck by MV#1. MV#2 sustained minor damages to its front passenger's side bumper. The owner of this vehicle was provided with the necessary information pertaining to this accident. MV#3 was parked in between MV#2 and MV#4 and was unoccupied. MV#3 was struck by MV#1 which pushed MV#3 into MV#4. MV#3 sustained heavy front end damages (Broken driver's side axle). The operator of MV#4 was inside his vehicle along with 3 other passengers when they were struck by MV#3. MV#4 sustained heavy damages to its driver's side rear door. There were no reported injuries to the operator of MV#4 or its 3 passengers. The witness stated he was inside his vehicle a few parking spots over from MV#4 and observed MV#1 drive over the burm striking MV#2 then striking MV#3 and pushing MV#3 into MV#4. MV#3 sustained the heaviest damage and was inoperable. MV#3 was towed by AAA Towing Services. I took digital photographs of the damages sustained to the city's burm.

On August 15th, 2021 at approximately 09:55 hours I along with NFD and Fallon Ambulance responded to a report of a single car crash into a stone wall at #144 Chapel St. On my arrival I observed a 2006 Brown Honda Accord, MA REG. 93ES83 crashed into a wall at #144 Chapel St. The operator was located outside the vehicle sitting in a chair inside the parking lot adjacent to #144 Chapel ST. She identified herself as Cleausa Cobden, resident of #145 Chapel St,(across the street from accident scene). She stated she had just turned onto Chapel St heading S/B from Watertown St, heading home when she got distracted for a moment taking her eyes briefly off the road and the next thing she remembers was crashing into the wall. She sustained minor injuries to her ankle and some minor cuts to her arm. She later signed a patient refusal for Fallon Ambulance. Tody responded and towed the vehicle. The property owner for the wall was later determined to be the property of the US Postal Office, #326 Watertown St. The wall sustained no damage from the impact that I could see.

On Sunday, August 22, 2021, I was contacted by Sgt. M. Wade of the Newton Police Traffic Bureau at 0032 hours. Sgt. Wade stated a serious motor vehicle crash occurred on Washington Street in Newton and it involved a City of Newton marked police cruiser (N494). Sgt. Wade stated Officer Katelyn Pohlman and the operator of the other vehicle involved were transported to the hospital with potential serious injuries. I was requested by Sgt. Wade to respond to the scene from my residence and investigate the crash further. On arrival at the scene, I observed the two vehicles involved in the crash to be a 2015 Ford Explorer (MA: 586RT7) and Newton Police marked unit N494 (MA MVN: MP494B). These vehicles were observed disabled on the roadway in front of 612 Washington Street, Newton. There was a large debris field comprised of various parts from both vehicles on the roadway and in the parking lot of 612 Washington Street. The road surface at the time of my arrival was wet and there was light rain falling. Sgt. Wade reported that at the time of the crash the road was completely dry and it had not started to rain yet. Washington Street is a public way maintained by the City of Newton. The portion of Washington Street where the crash took place supports two- way travel, Eastbound and Westbound. There are two travel lanes in both directions. Travel direction is divided by a double yellow line in the middle of the roadway. The area is well lit and features numerous light poles illuminating the roadway. The closest intersection to this location is Crafts Street just west of the crash location. MV1 (MA: 586RT7) had substantial front-end damage, especially to its front passenger side and was inoperable. I observed this vehicle at its final rest on the roadway with its front end facing Northbound on Washington Street just over the double yellow line in the middle of the roadway. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV1 was removed from the roadway by Tody's Towing. MV2 (MA MVN: MP494B) had substantial front end damage and was inoperable. The impact from the collision caused the vehicle to spin uncontrolled and the final rest, with the front end of the vehicle facing the front entrance to the 7-11 (S). The vehicle's final rest was on the sidewalk in front of 612 Washington Street. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV2 was removed from the roadway by Tody's Towing. The Officer involved in the crash is Officer Katelyn Pohlman. Officer Pohlman stated she was operating Newton Police marked unit N494 on Washington Street (E) past Crafts Street. Officer Pohlman stated as she was operating her vehicle past 612 Washington Street (7-11), a vehicle in the westbound lane of Washington Street abruptly crossed the double yellow line in the roadway and entered her travel lane head-on. Officer Pohlman stated she did not have an opportunity to react to the vehicle in front of her and the front passenger side of MV1 crashed into the front end of her vehicle (MV2). Officer Pohlman sustained injuries to her left forehead area and both of her arms. Officer Pohlman was later transported to Brigham and Women's Hospital for evaluation and was released a short time later. It was reported to me by numerous Officers on scene that Officer Pohlman exited her cruiser after the crash and rather than tend to her own injuries, she focused her efforts on rendering aid to the operator of the other vehicle involved. Officer Pohlman found the operator of the other vehicle lying on the floor of the front passenger seat under the dash. Officer Pohlman rendered aid to the operator while waiting for Newton Medics to arrive. Officer Pohlman was also assisted in rendering aid to the other operator by Officer Conary and Officer Larson. Officer Larson reported that while interacting with the operator of MV1, the operator repeated numerous times, "Is she ok? Is she ok?". Officer Larson also stated the operator said to him "I feel bad because I was texting on my phone". Officer Larson stated the operator of MV1 works for the Department of Corrections Special Operations Unit and has Department issues equipment inside the vehicle. The following items were placed into Support Services, locker 9 for safekeeping by Officer Larson: 1 Gray Pelican Gun Case, 1 Black Gun Belt, 1 Taser, 1 Can OC Spray, 15 (50) Round Cases Of Ammunition. The operator of MV1 (MA: 586RT7) was identified as Mr. David Cafarelli (S16965940). Mr. Cafarelli was transported to Newton Wellesley Hospital for a potential head injury before my arrival. I spoke with Mr. Cafarelli at Newton Wellesley Hospital in the Emergency Room with Sgt. Wade. Mr. Cafarelli's mother and father were present when we spoke with him. Mr. Cafarelli stated he was out with friends before the crash and was driving to the 7-11 on Washington Street. Mr. Cafarelli admitted that before the crash, he had his cell phone in his hand while he was operating his vehicle. Mr. Cafarelli stated he does not remember if he was texting or changing the music on his phone at the time of the crash. Mr. Cafarelli stated he looked up and was unable to avoid the vehicle (N494) directly in front of him. Mr. Cafarelli stated he also does not believe he applied his brakes before the collision. When we spoke with Mr. Cafarelli in the Emergency Room, he was still waiting for test results to determine the severity of his injuries. On Monday, August 23, 2021, Sgt. Wade responded to 612 Washington Street, Newton to retrieve surveillance footage from the 7-11 of the crash. 7-11 stated their outside surveillance system was not operating correctly, but they were able to capture some of the incident from a camera inside the store. I observed this footage. MV1 is observed traveling on Washington Street (W) in the westbound travel lane in front of the storefront. MV1's driver side is then observed traveling over the double yellow line in the middle of the roadway and into the eastbound lane of Washington Street. A head on collision ensues and MV1 can be seen rotating counterclockwise in the roadway backwards to its final rest location on Washington Street. MV2 is then observed at its final rest position on the sidewalk facing southbound in front of the 7-11 storefront. After completing my investigation, I have surmised the following. Mr. Cafarelli was operating his 2015 Ford Explorer (MA: 586RT7) in the area of 612 Washington Street, Newton. Mr. Cafarelli was holding his cell phone in his hand while operating his vehicle. Mr. Cafarelli became distracted while he was operating his vehicle due to his attention being focused on his cell phone. Mr. Cafarelli traveled over the double yellow line from the westbound lane of Washington Street and entered the eastbound lane of Washington Street. Newton Police marked unit N494 was traveling on Washington Street (E) in the area of 612 Washington Street at this time. Mr. Cafarelli's vehicle crashed into the front end of Newton Police marked unit N494. Mr. Cafarelli was not wearing a seatbelt and as a result of the impact from the crash, he was ejected from the driver seat and his final rest inside the vehicle was in the flood of the front passenger seat. As a result of my investigation, Mr. Cafarelli was issued Massachusetts Uniform Citation for Chapter 90, Section 13B (Hands-Free Violation), Chapter 89, Section 4a (Marked Lanes Violation), and Chapter 90, Section 13a (Seat Belt Violation). Photos from the crash scene and the surveillance media from 7-11 capturing the crash were submitted to the IT Bureau.

She could only articulate that she heard the crash. she admitted to me that she was distracted by the radio. She stated to me that she was crying and upset listening to the news updates of the ongoing overseas events in Afghanistan. V1 was traveling northbound on Commonwealth Ave coming through the intersection at the time of the collision with V2. V2 operator; Chahine, Georges simply stated that he was on his way to work and hit coming through the intersection. V2 was traveling eastbound on Washington St coming through the intersection at the time of the collision. V1&V2 both required towing service due to damage as a result of the collision. Tody's Towing responded and towed both vehicles from the crash scene. There was airbag deployment in both vehicles. The operators were each wearing seatbelts and neither were injured as a result of the collision.

The pedestrian, identified as Yuanhua Wei stated that she was walking west on Derby St and attempting to cross Parmenter Rd in the cross walk when she was struck by MV#1. The operator of MV#1 stated that she was traveling east on Derby St and began turning left onto Parmenter Rd because in her words, she didn't observe the pedestrian. The operator stated as she turned through the intersection, she observed the front of her MV strike the pedestrian, the pedestrian dropped to the ground and the MV#1 pulled over to the right side of Parmenter Rd. When asked, the operator stated she were not distracted while operating her MV. When I arrived Mrs. Wei was sitting in the cross walk directly next to the north west corner of that intersection. When asked about her injuries, she stated her hip hurt, she showed me an abrasion on her right elbow/forearm and her lower body was sore. I also observed one of Mrs. Wei's sneakers was on the ground next to her. Mrs. Wei was sitting the entire tire time, did not appear to have any serious injuries and attempted to get up shortly after I arrived, but advised her to remain sitting. Paramedics evaluated Mrs. Wei for minor injuries at the scene and she chose not to be transported to a local hospital for her injuries. I did not observe any damage to MV#1, which the operator confirmed. I informed both parties that I was filing a crash report with charges pending.

On September 14th, 2021 at approximately 09:22 hours while working N491 I responded to the area of #227 California St for a report of a MV crash. On my arrival I located both involved vehicles. Vehicle #1 was a 2000 Toyota Avalon XL, Ct reg AK76968, operated by a Molly Parrott. She stated she was going W/B on California St when she came to a flashing yellow light and slowed/stopped because she thought it was about to turn red. A few seconds later she was rear ended by vehicle #2. Vehicle #2 was a 2011 Brown Chevrolet. Malibu, Ma reg. 9JB522, operated by a Dion Tribbett. He reported travelling W/B on California St when he had to go around onto the wrong side of the road due to a truck blocking the west bound side of California St. Due to that distraction of him looking at the truck as he went around it he didn't see that vehicle #1 had stopped just a little further down the street. His vehicle then drove into the rear of vehicle #1 that had stopped by Jasset St/ Forte Park entrance. Both operators sustained minor injuries and both refused to be transported to the hospital. On Tuesday, September 21, 2021, I received a call from Sgt. Mike Wade of the Newton Police Traffic Bureau. Sgt. Wade stated a serious MVA/Bicycle crash had just occurred at the intersection of Cherry Street and Derby Street in Newton and the bicyclist was a 10-year-old boy with a possible head injury. Sgt. Wade stated the Traffic Bureau was requested to respond by the Shift Commander and asked if I could respond to investigate the crash. On arrival, Officers reported to me that the cyclist involved, Benjamin Lavoie, was transported to Children's Hospital in Boston with a possible head injury. It was reported to them that Benjamin was traveling Southbound on Cherry Street towards Derby Street on his bicycle. Benjamin entered the intersection of Derby Street and crashed into the front passenger side area of a passing motorist that had a green light. Officers on the scene also reported that the motor vehicle and the bicycle involved had been moved from their original rest location. Cherry Street is a public roadway maintained by the City of Newton. It supports two way travel (N and S) from Washington Street (S) to Waltham Street (N). The travel lanes are divided by an unprotected double yellow line median. The posted speed limit for this street is 25 miles per hour. Derby Street is a public roadway maintained by the City of Newton. It supports two way travel (E and W) from Waltham Street (E) to Lexington Street (W). The travel lanes are divided by an unprotected double yellow line median. The posted speed limit for this street is 25 miles per hour. The weather at the time of the crash was clear and sunny. The road surface in the immediate area of the crash was dry. I spoke with the operator of MV1, Ms. Erica Pusateri (S61409866). Ms. Pusateri stated he was operating her 2021 Kia Seltos (MA: 8HC743) Westbound on Derby Street towards Cherry Street. Ms. Pusateri stated that she had a green light at the intersection and proceeded to cross Cherry Street to continue onto Derby Street. Ms. Pusateri stated halfway through the intersection she heard a noise and saw a bicyclist hit the front passenger side door area of her vehicle. The collision caused the glass window from the front driver-side door to shatter. Ms. Pusateri's vehicle controlled her vehicle to a stop and she observed a young boy lying on the roadway in the intersection behind her. Ms. Pusateri exited her vehicle, ran over to the injured bicyclist, and rendered aid to him by stabilizing his neck. Ms. Pusateri remained on scene and cooperated fully with Officers. I asked Ms. Pusateri if she was distracted at all while operating her vehicle prior to the collison. Ms. Pusateri stated she was not. I asked Ms. Pusateri if she could recall where in the intersection the bicyclist crashed into her vehicle. Ms. Pusateri stated the bicyclist crashed into her vehicle in the Northwest side of Derby Street just outside of the marked crosswalk in the intersection. Ms. Pusateri stated she knows it was in the street in the intersection because she observed a woman with a stroller on the sidewalk on the corner of Cherry Street at Derby Street on the Franklin Elementary School side and she had already travelled through the intersection passed the East side of Cherry Street. Due to the unknown injury status of the bicyclist, Ms. Pusateri's vehicle was towed to the Newton Police Station and secured in the garage area as evidence. Ms. Pusateri reported a small laceration on her right hand with no other injuries. I observed damage to the front passenger side fender/door area. There was no airbag deployment as a result of the crash. I observed glass through out the front passenger side compartment area of the vehicle from the window breaking. I contacted Benjamin' s parents, Cheryl and John, to check on the status of their son. Cheryl stated Benjamin did not suffer any life-threatening injuries and appears to have bumps, bruises, and a cut on his head. I asked if I could speak with Benjamin for his account of what happened at the crash. Benjamin stated he was riding his bicycle on Cherry Street (Southbound) on the sidewalk on the East side of Cherry Street. Benjamin stated he left the sidewalk and entered the intersection to cross Derby Street and continue on Cherry Street. Benjamin acknowledged that at this time Cherry Street had a red light and Derby Street had a green light. Benjamin stated he then crashed into a passing car in the roadway. Benjamin was wearing a helmet at the time of the crash. The helmet had significant damage to it as a result of the collision. I told Benjamin his bicycle was being held at the Newton Police Department for safekeeping and we will arrange to get it back to him. Benjamin is a student at the Franklin Elementary School. I make contact with a witness to the crash, Mr. Benjamin Crosby. Mr. Crosby stated he did not witness the actual crash, just the aftermath. I was able to reviewthe 9-1-1 call Mr. Crosby made regarding the crash in Newton Disptach. Mr. Crosby stated a boy on a bicycle was in a crash with a motor vehicle. Mr. Crosby is heard saying "he went through the light" to the Dispatcher taking the call. After completing my investigation, I have summized the following. The bicyclist (Benjamin Lavoie) traveled down Cherry Street (S) and went through a red light to cross the intersection at Derby Street (W). As a result of traveling through the red light and entering the intersection, he crashed into the front passenger side door area of a passing motorist that had a green light (Erica Pusateri). No citation will be issued as a result of this crash.

I observed lighter scratches and gouges to the rear left side quarter panel of the vehicle, the bumper was knocked off,but was recovered and still operational. -There were no injuries reported at the scene and neither vehicle was towed.

Narrative:
On Sunday 6/12/22 at approximately 1333 hours while assigned to marked unit n498 I was dispatched to the area of 832 Dedham Street for a report of a 2 car motor vehicle crash with no injuries. Upon arrival I spoke with the operator, GERSHON Andrew, of motor vehicle #1 (6HS292). GERSHON states he was traveling Westbound on Dedham Street toward Brookline Street when he approached the driveway at 832 Dedham Street and motor vehicle #2 started to pull out in front of him. GERSHON states as he saw the vehicle pull out he swerved into oncoming traffic to avoid the collision, however motor vehicle #2 struck the passenger side of his vehicle. After the collision GERSHON attempted to pull back into his lane of travel, but lost control causing him to overcorrect and go off the road on the westbound side of the road in front of 824 Dedham Street causing some damage to the front landscaping of the yard. Secondly, I spoke with the operator, JIN Zhien, of motor vehicle #2 (EV87SA). JIN states she was attempting to pull out of the driveway of 832 Dedham Street and didn't see motor vehicle #1. JIN states she was distracted because there was a lot going on inside the car. She stated she was not on her cell phone. Finally I also spoke with a witness, CURTIS Ron, who was travelling in the opposite direction and witnessed the crash. Ron stated he saw motor vehicle #1 travelling straight in the opposite direction on Dedham Street. He saw the Tesla (MV#2) attempting to pull out of the driveway at 832 Dedham Street and drive into the side of MV#1. Ron stated he also observed MV#1 swerve into his lane of traffic trying to avoid the collision and then spinning out into the front of 824 Dedham Street. Taking the witness, operator, and passenger statements into account as well as my observations, I determined the operator, (JIN Zhien) of MV#2 (EV87SA) is at fault for the crash and she has been issued Massachusetts Uniform Citation T2012918 C89 S8 Failure to Yield Right of Way.

Narrative:
Operator of Vehicle 1 states he was travelling Northbound on Ferncroft Rd when the operator became briefly distracted by his phone. Vehicle 1 then struck the curb and a tree on the West side of the road in front of #64 Ferncroft Rd, causing extensive damage to the front of the vehicle and airbag deployment. The operator denied any injuries and declined medical treatment. Vehicle 1 was towed from the scene by Tody's Towing. Newton Fire responded to clean up engine fluids that had leaked on the roadway. The witness who initially reported the crash to 911 stated that she observed the operator of Vehicle 1 pass her location at a higher than reasonable rate of speed and was looking down at his phone. I then issued the operator Mass. Uniform Citation #T1448099 (Written Warning) in hand to the operator's mother as his agent for a violation of C90ss13B Texting While Driving

Narrative:
Operator of MV1 (MA reg. 54W780) states she was traveling eastbound on Washington Street when she approached the traffic light which was yellow and turning red. Operator of MV1 states she came to a stop at the light and a short moment later was struck from behind by MV2. Operator of MV2 (MA reg. 673YZ4) states he was traveling eastbound on Washington Street behind MV1. Operator of MV2 states he was distracted looking back at the rear passenger in his vehicle. When Operator 2 looked back at the road he was unable to stop and struck MV1. All parties stated they were not injured at the time. I issued all motorists involved the proper information for insurance purposes. Operator of MV2 was issued MA. uniform citation (T0644896) for following too closely.

On Monday, July 18, 2022, while assigned to unit N493, I responded to the area of 1750 Commonwealth Avenue, Newton for a reported crash. The weather at the time of the crash was overcast and cloudy. The road surface was dry. Commonwealth Avenue is a public way maintained by the City of Newton. The operator of MV1, Ms. Elizabeth Fabrizio (S29560661) stated she was stopped at a red light in her 2012 Acura MDX (MA:157SE7) on Commonwealth Avenue (W) just before Washington Street. Ms. Fabrizio stated while stopped at the light, MV2 crashed into the rear of her vehicle. I observed damage to the rear bumper area of MV1. Ms. Fabrizio and her passenger, Ms. Mary Daley, reported no injuries. The operator of MV2, Mr. DJ Kirby-Thomas (S32655458) stated he was operating a 2009 Chevrolet Silverado (MA CON: S87959) on Commonwealth Avenue (W) towards Washington Street. Mr. Kirby-Thomas stated while operating his vehicle he became distracted by the GPS in his vehicle and his front end crashed into the rear of MV1. I observed damage to the front bumper area of MV2. Mr. Kirby-Thomas reported no injuries. Mr. Kirby-Thomas was provided with Massachusetts Uniform Citation 519268AB in hand for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care).
