OPMV#1 Was traveling Northbound on Grove St when she came to a stop at the intersection of Grove St and Woodland Rd. OPMV#1 Stopped and then proceeded into the intersection and was struck by the OPMV#2. OPMV#1 Stated she stopped at the stop sign and was waiting for the other vehicle to go but they didn't move. So she started to move into the intersection and that's when she was struck by the OPMV#2. OPMV#2 Was traveling Westbound on Woodland Rd when she came to a stop at Woodland Rd and Grove St. OPMV#2 Stopped and then proceeded into the intersection and was struck by the OPMV#1. OPMV#2 Stated she stopped at the stop sign as the other vehicle did and happened to be entering the intersection at the same time when she was struck by the OPMV#1. Vehicle#1 was towed due to extensive damage. There were no injuries due to this accident.

MV1 was traveling southbound on Walnut St when the operator, Saptarshi Ganguly, stated that he began to feel fatigued, believed he blacked out and then woke up after impact with MV2. Mr. Ganguly stated that he takes Atenolol for vasovagal syncope. I know this to be a condition that makes individuals feel like they are going to faint. He stated he was diagnosed with the disorder about 10 years ago. MV1 crossed over the double yellow lines on Walnut St and collided head-on with MV2 who was traveling northbound on Walnut St. Two witnesses (Kyle Fieleke and Edward Aybar) at the scene corroborated the story and said that MV1, in a 2015 BMW X535I, MA Reg: 668BG5 veered into the opposite lane and hit MV2 head-on, a 2018 Tesla Model3, MA Reg: EV585N. MV1 operator, Saptarshi Ganguly, was issued MA Uniform Citation T1440468 for 89/4a for Marked Lane Violation. A Request for Medical Evaluation form was filled out for Saptarshi Ganguly and faxed to the MA RMV at 857-368-0018 as I am unable to determine his driving ability. Both operators were transported to Newton Wellesley Hospital to be evaluated. Both vehicles had extensive front end damage and were towed by Tody's towing.

MV1 was traveling SOUTH on Cherry St through the intersection of Webster St. MV2 was traveling WEST on Webster St through the same intersection. MV2 collided with MV1 inside of the Cherry St/Webster St intersection causing heavy front end damage to MV2 and damage to the rear drivers side wheel and fender. MV1 operator, Leah Wilson, stated that she had a green light and proceeded through the intersection when she was hit by MV2. MV2 operator, Caroline Steiner, and her 12yo daughter (passenger/front seat) Lorelai Steiner, stated that they were proceeding through the green light and observed MV1 going at a high rate of speed through the intersection. No injuries reported. Both vehicles towed by Tody's. MV1 operator, Leah Wilson was issued in hand MA Uniform Citation T1440472 for 89/9 - Red Light/Fail. On scene, I observed debris surrounding the four-way intersection and MV1 laid to rest past the intersection and facing the wrong direction in traffic. The damage to both vehicles was severe and MV1 had air bag deployment. The reporting party was an NPD auxiliary officer who had responded prior to my arrival because he heard the crash from the backyard of the police station which is approximately 350-400ft. Based on my training and experience, I believed Leah Wilson to be traveling through the intersection while failing to yield to the traffic control device.

On 02/08/2019, I, Officer Conary, responded to 309 Tremont Street for a past hit and run involving a pedestrian. Upon arrival, I was met by FEBLOWITZ, Colleen, who stated at approximately 1745 hours, she was crossing in the crosswalk at Tremont at Hibbard when her foot was hit by a car. FEBLOWITZ explained to me that she was in the crosswalk and she saw a silver sedan, MV1, traveling Eastbound on Tremont Street approaching her but it was far away where she felt safe to walk across the street. She said she also raised her hand to draw attention to herself. FEBLOWITZ was halfway in the crosswalk when she felt severe pain in her right foot. She looked behind her and saw MV1 was very close to her. MV1 continued to drive down Tremont Street and not stop. There were no witnesses. FEBLOWITZ went to Partner's Urgent Care to be treated for minor injuries. The crosswalk is in a lighted roadway. FEBLOWITZ was unable to gain any other information on MV1. No further incident to report.

Narrative:
On Friday March 15th 2019 at approximately 0730 hours I responded to the intersection of Beacon Street and Herrick Road for a report of a hit and run involving a bicyclist. The weather at the time of the accident was cloudy and the road conditions were dry. Both streets are public ways in the City of Newton. Upon arrival I observed the victim identified as Glenn Butcher (MA OLN S36552335), inside the ambulance getting medical attention. Butcher was getting a bandage applied to his left leg and complained of leg, back and neck pain. Butcher appeared visibly shaken and in pain. Butcher stated he had the green traffic light and was travelling eastbound on Beacon Street crossing Centre Street and then continuing straight crossing Herrick Road. Butcher stated MV1 was travelling westbound on Beacon Street and took a left turn onto Herrick Road and collided into his bicycle. Butcher states upon impact he flipped over his bicycle and landed on the hood of MV1, before rolling off and landing on the ground. Butcher states he was wearing his helmet which was present. Butcher states his bicycle ended up partially under MV1. Butcher states the operator of MV1 pulled his vehicle to the side of the road and got out of his vehicle and spoke to several witnesses briefly. Butcher states the operator then left the scene without properly exchanging information and did not wait for the police to arrive. Butcher described the vehicle as a dark gray Honda, and described the operator as an older white male, 5'10 approximately 80 years old with white hair. Butcher was also able to provide an email address for another bicyclist who witnessed the accident. The witness left his information with Butcher prior to my arrival. Fire fighters on scene were able to get the registration number of the vehicle from the witness (MA REG 4CX148) and gave it to Officers on scene. Butcher was transported by Medic 1 to NWH with minor injuries. His bicycle had visible damage to the frame area and skid marks were present on both tires. 19 photographs of the scene and the bicycle were taken and sent to the IT dept. email. Officer Gabriel transported the bicycle to the station where it was logged into Property & Evidence for safekeeping. Dispatch was able to query the registration which came back to a gray, 2010 Honda Accord registered to a Matthew Sugarman (MA OLN S38601397), out of W. Roxbury. Dispatch contacted the Boston Police Dept. and had them send an Officer to the address to try to make contact with Sugarman. Sugarman was advised to contact the Newton Police Dept. and I was able to speak with him on the telephone. Sugarman states he was driving the vehicle today and was involved in an accident with a bicycle. Sugarman states he was travelling westbound on Beacon Street and was taking a left onto Centre Street not Herrick Road. While speaking with Sugarman he seemed confused as to his location during the accident and was adamant that he was turning left onto Centre Street and not Herrick Road. I advised Sugarman that I was on scene with the first responding Officers and the accident clearly occurred at the intersection of Beacon Street and Herrick Road based upon the final resting place of the bicycle. Sugarman went on to state that his vehicle was already turning when the bicycle struck his vehicle and he believed it was the bicyclists fault. I advised Sugarman it appears the bicyclist had the right of way since it was traveling straight and that because his vehicle was taking a left turn, he failed to yield for on coming traffic at an intersection. When asked why he did not stay at the scene of the accident Sugarman stated he got out of his vehicle and spoke to the bicyclist and he "seemed okay". Sugarman states he then left the scene while the victim was still sitting on the ground. I asked Sugarman if he had ever been involved in accident before and he replied "no." I advised Sugarman that I am currently looking at his drivers history and see nine accidents that he was involved in. Sugarman stated "he has never been in accident like this before." I then asked Sugarman what the law states when you are involved in an accident and he was not able to provide me with an answer. I advised him that the law states you must stop and exchange information before leaving the scene of an accident. Again he stated the bicyclist seemed okay and "he had somewhere to be so he left." Sugarman has an extensive drivers history including nine surchargeable accidents, reckless operation, two different charges of leaving the scene property damage and two immediate threat affidavits filed against him. I advised Sugarman that I will be mailing him a citation for Ch89 s8 Fail to yield to oncoming traffic, and Ch90 s24 Leaving the scene of personal injury. I also filed an immediate threat form with the registry. I emailed the witness using the information he provided to the Newton Fire Dept that was on scene and received a phone call back from the witness identified as Bruce Adams. See witness report for his statements. I contacted the Bank of America located at 808 Beacon Street in Newton, as they are the closest business to that intersection with video surveillance. I spoke with George Walsh (781-834-8275) who stated they do not have any working cameras on that side of the building to the bank. For this report the bicyclist is entered as a non-motorist for validation purposes. Witness Bruce Adams (MA OLN S96398108) stated that he was riding his bicycle travelling behind the victim about 100 feet away. Adams states he was watching the victim and had a clear view of him as he travelled behind him, through the green traffic light heading eastbound on Beacon Street. Adams stated he witnessed MV1 travelling westbound on Beacon Street take a sharp left turn onto Herrick Road which turns into Union Street. Adams stated that the vehicle turned left colliding into the bicycle and there was no possible way the bicyclist had time to stop to avoid the collision. Adams stated the victim flew over his handle bars and fell on top of MV1's hood and then landed on the ground. Adams stated the victim's bicycle was stuck under MV1's front bumper. Adams stated the operator of MV1 backed his vehicle up with the bicycle still attached to his vehicle bumper and pulled MV1 over to the side of the road. The operator then got out of his vehicle and walked up to the victim who was laying on the ground visibly in pain. Adams stated the operator asked the victim if he was okay to which the victim did not immediatley reply as he was visibly shaken up. Adams stated the operator then walked back to his vehicle and attempted to leave the scene and that is when Adams stood infront of MV1 to stop him from leaving the scene. Adams stated he then removed the bicycle from underneath the bumper of MV1 and placed it on the side of the road. Adams stated the operator got out of his vehicle again and walked up to the victim and asked him "what do you want to do?". Adams stated the victim did not respond and that is when the operator got back into his vehicle and drove away, leaving the scene before any emergency response arrived. Adams stated he was able to memorize MV1's registration and stayed with the victim until police, fire and medics arrived. He stated he gave the fire dept the registration and his contact information prior to leaving the scene.

On 03/19/19 at 14:57 hours, I responded to 220 Dedham Street for a past motor vehicle hit and run accident involving a parked car that occurred at 12:26 hours on this date. Dedham Street is a public way maintained by the City of Newton. The owner of M/V # 1 states her vehicle was legally parked in front of 220 Dedham Street facing northbound when she came out and found a note from a witness saying it was struck by a gray Honda Accord bearing MA Reg. # 8512HV. It appears that the operator of M/V # 2 side swiped M/V # 1 as it was passing by on Dedham Street. I observed marks to the driver's side rear door area and the side view mirror was broken. On 03/20/19 at 08:47 hours, I spoke to the witness, Kawtar Bhih, on the phone and she stated that she was parked on the opposite side of where the accident took place. Bhih observed a gray Honda Accord with an elderly female operating M/V # 2 on Dedham Street almost side swiped another parked car before side swiping M/V # 1. She says that the vehicle slowed down after and then continued to leave the scene of the accident. She then took a photo of the above listed registration plate. I attempted to contact the owner of M/V # 2 (Jali Medical Inc Waltham, MA) and the receptionist was not able to provide any information at the time. I was told to call back the next day. I gave the vehicle and registration info to the person inquiring if they could find a possible driver and call back. While working on this report, I observed that the owner of M/V # 2 was involved in a past accident in our system and the operator at the time had an address of 66 Country Club Rd. Newton. On 03/20/19 at approx. 09:30 hours, I went over to this address and observed M/V # 2 in the driveway with an operator inside. After inquiring about the accident, she stated she was driving down Dedham Street when she heard a noise coming from her passenger side view mirror as she had struck something. She then left the scene not thinking it was serious. I observed fresh damage to her passenger side mirror along with other marks along the doors which appeared to be older. Operator was issued in hand MA Uniform Citation # T1441710 for Ch. 90/S24 Leaving the scene of property damage.

VEHICLE #2 STATES SHE WAS TRAVELING S-BOUND ON CHERRY ST. AS SHE APPROACHED WEBSTER ST. SHE STATES SHE WAS BLINDED BY SUN GLARE AND CONFUSED THE LIGHTS AT CHERRY AND WASHINGTON AS BEING THE CORRECT LIGHTS ( THEY WERE GREEN AT THE TIME ). WHEN SHE REALIZED THE LIGHT AT CHERRY AND WEBSTER WAS RED SHE WAS UNABLE TO AVOID HITTING VEHICLE #1. VEHICLE #1 HAD EXTENSIVE REAR AND LEFT FRONT END DAMAGE AND WAS TOWED BY TODYS. VEHICLE #2 HAD EXTENSIVE FRONT END DAMAGE AND WAS TOWED BY TODYS. OPERATOR OF VEHICLE #2 WAS MAILED CITATION ( T1442287 ) FOR CHAPTER 89/9 FAILURE TO YIELD. VEHICLE #1 WAS MAILED CITATION ( T1442286 ) FOR PASSENGER RESTRICTION VIOLATIONS UNDER JUNIOR OPERATOR LICENSE. ALL PARTIES , ( WITH THE EXCEPTION OF WRIGLEY LOWE ) REPORTED NO INJURIES AND SIGNED PATIENT REFUSALS. CHRISTOPHER CAMPBELL AND SERGIO ALVAREZ WERE TURNED OVER TO THEIR PARENTS. WRIGLEY LOWE WAS TRANSPORTED BY MEDIC2 TO NEWTON WELLESLEY HOSPITAL FOR MINOR CUTS. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES. The operator of vehicle 1 stated that with a green left turn arrow showing, and her left directional on she went to turn onto Waltham Street from Crafts Street when all of the sudden she hit a bicyclist. She stated she did not see the bicyclist at all prior to him hitting her car. The bicyclist stated does not remember anything from the accident at this time. The bicyclist was traveling southbound on Crafts Street and appears to have entered the intersection of Crafts and Waltham Street without stopping and while a red light was showing. He was wearing a helmet at the time of the crash, he was not wearing any reflective clothing(dark green jacket), and the bike did not have any lights. The bike was taken to support services for safe keeping by Ofc. J.March. Vehicle#1 was towed away by Tody's towing due to extensive damage. Bicyclist was transported to Beth Israel hospital for several trauma injuries. Pictures of scene was taken by me and sent to IT Bureau. Both witness's statements were the same. They stated that they were behind vehicle 1 in traffic and saw that she had on her left dirrectional signal and had a green left arrow signal when vehicle 1 went to turn left onto Waltham Street.

This caused moderate damage to the front of her vehicle. She was evaluated by Cataldo and signed a medical refusal and was able to drive her car away. The Operator of vehicle 2 stated she was traveling North on Bullough Park, she came to a stop at the stop sign and proceeded to cross over Mill Street towards Upland Road. She was then hit on the passenger side by vehicle 1. She stated she did not see vehicle 1 at all. As a result of the impact, vehicle 2 sustained extensive damage to the passenger side and had side air bag deployment. The passenger in vehicle 2 stated he did not see anything until impact. The Occupants of vehicle 2 stated they had no injuries or pain and were taken away from the scene by parent Niels Steenstrup. As a result of the accident vehicle 2 was towed by Tody's by Billy.

On April 24, 2019, while assigned to N496, I was called by Dispatch to respond to the area Beacon Street and Hammond Street for a motor vehicle crash. This was called in by Boston College Police Department. Upon arrival MV #1 was on Beacon Street just past Hammond Street at the Island with considerable vehicle damage. MV#1 stated while traveling westbound on Beacon St, while approaching the intersection of Hammond he stated he started to feel strange and not like himself. MV#1 stated he then struck the curb with his front right tire, not believing his accident was very serious at this time. He reversed his vehicle off the sidewalk and proceeded to travel westbound on Beacon St. After passing through the intersection he stated his car felt very off at which point he initiated his hazards and pulled over to the right. MV#1 stated he has a history of epilepsy and currently takes Oxcarbazepine twice a day for his condition. He stated he did not believe he had a seizure or lost consciousness but did not recall the incident very well. Boston College Ofc. Cadogan witnessed the incident, and stated the following, while conducting a directed patrol at the intersection of Beacon and Hammond St he saw MV#1 traveling westbound on Beacon St. Ofc. Cadogan stated MV#1 then took a sudden tight turn on to the curb striking the metal fence surrounding the BC facility. MV#1 was then seen / heard revving his engine while still in drive before reversing his vehicle off the curb and attempting to continue on Beacon St westbound. The fence surrounding the BC Facility sustained moderate damage. Tire marks in the grass show the path of MV#1 in direct line to the damaged property. While on scene, I spoke at length with Vanhorne. I did not detect or any alcohol from his breath as he spoke. Vanhorne reported he only consumed one beer tonight at roughly 2030hrs and smoked a little marijuana at 2200hrs. Vanhorne's eyes did not detect any signs of alcohol and he walked with no signs of any impairment. Sgt. Devine administered a horizontal nystagmus test which revealed no signs of impairment. Vanhorne reported he was at a work event and then visited a friend in Brookline. Vanhorne reported his last seizure from epilepsy occurred a little over two years ago. Pictures of the scene were taken and submitted to IT to be attached to this report. MV#1 operator stated he was not injured at this time. MV#1 was towed from the scene by Todys's due to heavy front end damage. Vanhorne was picked up from the scene by Paul Miller his step father. A towed MV form was filled out and placed in the traffic box. Witnesses Cassandra Williams and Bill Peirce observed the crash and stated identical narratives to Ofc. Cadogan's. An Immediate Threat form was filed with the RMV (via Fax and E-mail at approximately 0650hrs 4/24/19)due to his medical history and his explanation with this crash. All streets mentioned are public ways in the City of Newton. It should be noted that the road was wet from rain on this night. Update: on 4/27/19 Sgt.Devine was able to watch the video of the incident from Boston College Police. The video confirmed the statements given by BCPD and witnesses's. Due to the aforementioned statements and the video surveillance of the incident, MV#1 Operator will be cited with Marked Lanes Ch89/4a (MA Uniform Citation #T1441792. Vanhorne will be notified via phone he will be receiving the ticket in the mail.

At approximately 1825HRs on August 06, 2019, I was dispatched to 785 Centre St. (Newton Country Day School) in reference to a motor vehicle vs bicycle crash. Upon arrival, I located both parties on the sidewalk adjacent to the intersection of Centre St. and Colby Rd. The operator of the bicycle was complaining of feeling dizzy, although she stated she did not hit her head, and suffered several abrasions on her left leg and hands; the operator of MV1 stated she was uninjured. The operator of MV1 stated that as she was traveling northbound on Centre St., she attempted to give the operator of the bicycle room, but the bicyclist made an abrupt left hand turn and struck MV1. The operator of the bicycle stated she was traveling northbound on Centre St. in the Bicycle Lane. She then signaled a left hand turn and entered the Motor Vehicle Travel Lane to make a second left hand turn to travel eastbound on Colby Rd. Once she exited the northbound Motor Vehicle Travel Lane on Centre St. and began to cross over the southbound lane, MV 1 crossed over the solid yellow lines onto the southbound side of Centre St. and struck her bicycle. MV1 sustained damage to its right front bumper and I noted red paint transfer of the left side of the front bumper, which the operator of MV1 states was old damage. MV1 also sustained a flat left front tire after contacting the curb. The bicycle sustained damage to both tires and where the rear tire attaches to the frame appeared to be warped. I took several photos of the damage and have submitted them to be attached to this report. The operator of the bicycle was transported to the hospital by Newton Medics and the operator of MV1 signed a patient refusal. Todys Services arrived and took custody of MV1, for which I completed and filled an NPD Towed Motor Vehicle Form. All streets mentioned are public ways in the City of Newton. Due to the potential danger of serious bodily injury caused by the operator of MV1's reckless driving behavior (crossing the solid yellow road divider line and driving on the wrong side of the road against traffic), the actual damage caused (striking and causing the bicycle operator to be transported to the hospital and damage to the bicycle) and that the bicycle operator was clearly visible/in clear contrast with the road (riding a red bicycle and wearing a red shirt) I issued the operator of MV1 MA Criminal citation T1445010, in hand, for violating one count of MGL Ch.90 S. 24 (Operating To Endanger) I cleared without incident. Note: I logged the bicycle into property/evidence for safekeeping, as it could not be taken on the ambulance.

MV1 (2010 Honda Fit; MA Reg: 94CY78) was traveling East on Crafts St, and came to a complete stop signaling to turn left onto Morrill St. MV2 (2015 Ford F250; MA Con: R43023) rear ended MV1, resulting in severe damage to the Honda. Brian spoke Spanish and a passerby was able to translate some of my questions to him. I asked for identification and if he had been drinking. On Saturday, September 28, 2019, I responded to the intersection of Crafts Street and Albemarle Road, Newton with Sgt. Lee for a report of a motor vehicle crash involving a motorcycle with serious injury to it's operator. The weather at the time of the crash was clear and sunny. The road surface was dry. Crafts Street and Albemarle Road are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jussara Pontes (S92930243). Pontes stated she was operating a 2017 Toyota Sienna (MA: 4GX777) at the time of the crash. Pontes stated she was traveling North on Albemarle Road and came to a stop at the stop sign located at the Crafts Street intersection. Pontes stated she looked both ways and began to inch out into the roadway. Pontes stated she stopped the vehicle again and was assisting a child sitting in the 2nd row of the vehicle. Pontes stated she then turned her attention back to the roadway. Pontes stated she looked to her right and left again and did not see any vehicles in either direction. Pontes stated she proceeded to cross the intersection to continue North on Albemarle Road. Pontes stated the front bumper area of MV1 crashed into the left side of a passing motorcycle. Pontes stated the motorcycle and it's operator fell to the roadway and slid across the street to the side of the road. Pontes stated she pulled her vehicle over onto Albemarle Road. Pontes stated she did not see the motorcycle prior to the crash and was not distracted. The operator of the motorcycle (MV2), Irene Pistoftzian (S06099449) was transported to Brigham and Women's Hospital with a serious left leg injury prior to my arrival. I observed a red 1990 Kawasaki ZR550 (MA MCN: 1V1063) resting on it's right side on the NE sidewalk area of Albemarle Road at Crafts Street with it's front wheel/handlebar facing Crafts Street. The motorcycle had moderate damage to it's left side from the impact of MV1 crashing into it. The right side of the motorcycle had damage from sliding on the roadway and the crashing into a fence on the sidewalk on Crafts Street (W) at Albemarle Road. There was also damamge to the front headlight/right handlebar area. The motorcycle and Toyota Sienna were towed from the scene by Tody's towing at Sgt. Lee's request. Pictures were taken of the scene and submitted to the IT Bureau. I spoke with Pistoftzian at Brigham and Women's Hospital in Boston on Monday, September 30, 2019. Pistoftzian stated she was operating her motorcycle on Crafts Street (W). Pistoftzian stated as she approached the intersection of Crafts Street and Albemarle Road, she observed a mini van to her left out of her peripheral. Pistoftzian stated she remembers being hit on her left side and then laying on the roadway looking up at the sky on the sidewalk with her head facing Albemarle Field. Pistoftzian stated as a result of the crash she has a significant injury to her left leg and also has an injury to her left wrist/arm. Pistoftzian stated she was wearing a helmet at the time of the crash. I spoke with Kymberley Mitchell via telephone who stated she witnessed the crash. Mitchell stated she was traveling behind the motorcycle involved in the crash on Craft Street (W) at North Street. Mitchell stated they were stopped at a red light at this time. Mitchell stated there were no vehicles in front of the motorcycle at the light. Mitchell stated the light turned green and they continued on Crafts Street at a low rate of speed. Mitchell stated she could see ahead at the intersection of Albemarle Street and Crafts Street, traffic was stopped on Crafts Street (E) letting traffic exit Albemarle Road (N). Mitchell stated vehicles were taking left and right turns out of Albemarle Road before the motorcycle reached the intersection. Mitchell stated a mini van was crossing past the double yellow line of Crafts Street to continue straight when it crashed into the left side of the motorcycle. I spoke with Cheryl Wermer via telephone who stated she witnessed the crash. Wermer stated she was traveling on Crafts Street (E) towards Albemarle Road. Wermer stated traffic ahead of her was stopped to allow vehicles to turn off of Albemarle Road. Wermer stated she observed a motorcycle traveling towards her on Crafts Street (W). Wermer stated she then saw a vehicle travel straight from Albemarle Road across Crafts Street and crash into the motorcycle. Wermer stated the operator of the motorcycle flew off the bike. Wermer stated the car then came to a stop and the driver got out and stated "Oh my god! I didn't see you!" Wermer stated both vehicles were traveling at a low rate of speed prior to the crash. I spoke with Bill Frabizio via telephone who stated he witnessed the crash. Frabizio stated he was behind the mini van (MV1) on Albemarle Road prior to the crash. Frabizio stated the vehicle in front of him began to cross the intersection as he was monitoring the traffic on to take a right turn onto Crafts Street. Frabizio stated he heard a loud "Boom" and saw a motorcycle slide across the street with its operator. Frabizio stated the motorcycle crashed into a fence post and the operator came off the motorcycle. Frabizio stated he pulled over and assisted operator of the motorcycle. After concluding my investigation, the operator of MV1, Jussara Pontes, was mailed Massachusetts Uniform Citation T2016424 for Newton City Ordinance Chapter 19, Section 75 (Failure To Use Care).

Daigle Electric notified and responded. Vehicle#1 was towed by tody's due to extensive damage. There were no injuries due to this accident.

Operator #1 states he was driving northbound on Lexington St operating MA reg 47C440 (vehicle #1). Once at the green light at Lexington/Rumford Ave, operator #1 attempted to take a left turn onto Rumford when MA reg 4FR581 (vehicle #2) collided with him. Vehicle #2 was driving eastbound on Rumford and operator #1 states he did not see the car until the crash occurred. Operator #2 reports he was driving eastbound on Rumford and was attempting to take a right turn onto Lexington St at the intersection. Operator #2 acknowledges that he had a red light but states he stopped prior to turning (right on red is allowed). It should be noted that upon my arrival on scene, vehicle #2 was located partially in the intersection with airbags deployed and I am not able to conclude that vehicle #2 was in fact turning right. The point of impact does appear to be the left front area. Operator #2 states he doesn't know what happened, and that vehicle #1 may have oversteered the left turn. The damage to both vehicles is extensive and mostly contained to the front end/undercarriage areas. Both vehicles were towed by Tody's and all parties signed patient refusals from medics on scene.

I spoke to the operator of the Ford Focus John KELLY who stated he was taking a left out of the parking lot of 2102 Commonwealth Av, traffic in the EB lanes stopped to let him out, as he was entering the WB lane he collided with the pick up. Ford Focus towed by Todys, both operators signed refusals with Cataldo.

At approximately 2250HRs on Friday, December 13, 2019, I was dispatched to the area of 275 Washington St. for a report of a hit and run crash; Newton Officers and a State Police Officer canvassed the area with negative results. Upon speaking with the operator of MV1 (MA Pass:1CTB36), he stated that he was traveling Westbound on Washington St., when he observed a black Honda mini van or SUV, MV2, approach him in his passenger's side mirror and then attempt to enter his lane of travel, striking the rear of MV1. Upon speaking with the two witnesses who were traveling behind MV1 and MV2, they concurred with the operator of MV1's account. The witnesses described MV2 as a black Honda or Toyota mini van or SUV. I observed damage to the right rear quarter panel, trunk area and right rear tire of MV1. Todys Services arrived and took custody of MV1. I filled an NPD Towed Motor Vehicle Form. **Update as of 1800HRs on 12/15/2019** I was made aware that the operator of MV2 in this incident, now identified as MA Pass: 8HD155, submitted a Commonwealth of Massachusetts Motor Vehicle Crash Operator Report on 12/14/2019. In said report, the operator admits to striking MV1 and stated that he pulled over on Peabody St., where he remained for approximately 15 minutes awaiting MV1. When MV1 did not arrive, the operator of MV2 attempted to drive away, but realized the damage was too severe and returned to the same spot on Peabody St. I viewed MV2 at Clay Auto Body and observed damage to its left fender/wheel well area, as well as a deployed passenger's side airbag. Upon speaking with the operator of MV2, he reiterated the same information he stated in the above summary of his Commonwealth of Massachusetts Motor Vehicle Crash Operator Report. In the presence of his parents, I advised him on the proper steps to take when involved in an accident, such as the proper exchange of information and the potential ramifications of not doing so. Due to the operator of MV2 being 17 years old, being issued his driver's license 9 months ago and his evident remorse/confusion surrounding the incident, I will not be seeking any charges. I updated the operator of MV1 and provided both parties with an NPD Crash Data Exchange Form.

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

On Wednesday 6/17/2020 at approx 2116hrs, while assigned to N-494, I responded to the intersection of Walnut St and Comm Ave for MVA involving a bicyclist. The bicyclist, Aziz Boltayev, said he was traveling NB on Walnut St and attempted to cross Comm Ave. He stated that he was in the middle of the intersection when Comm Ave' light signal turned green. Contact was made to MV1s driver side front panel. Boltayev was transported to Beth Israel with serious injuries to his legs. Boltayev was not wearing a helmet nor wearing reflective clothing. His bicycle, a blue Schwinn, had no front head lamp attached in violation of MGL c85 s11B. A citation was issued and mailed to Boltayev. He was wearing a light colored shirt and shorts. Operator of MV1 states that he was on Comm Ave heading WB and stopped at a red light at the intersection of Walnut St. He states that he had a green signal when attempting to cross the intersection and did not see the bicyclist prior to the accident. Pictures were taken and submitted to IT. The Schwinn bike was placed in property and evidence for safekeeping.

Reportedly STB June 25 at 1500hrs and June 26 at 0700hrs, an unknown Toyota MV struck a large wooden fence structure and struck a large back up generator behind that fence and left the scene at the Adelaide of Newton Centre grounds at 157 Herrick Rd. On Wednesday, 7/1/2020, at 2230hrs, I was dispatched to the area of 856 Watertown St for a report of a possible car crash. Watertown St is a public way as it is owned and maintained by the City of Newton. Upon arrival, I met with operator, Henok Kiflit, who was out of the vehicle and very disorientated. I observed a gash on his head right above his eye and noticed all airbags had been deployed in his vehicle. I asked him to sit down and I radioed for an ambulance. Kiflit stated that he was driving EAST on Watertown St when he collided with a unoccupied parked car (a red 2015 Jeep Wrangler, MA Con R22519) which was parked on the street outside of 856 Watetown St. Kiflit was driving his girlfriend's car (a gray 2020 Nissan Sentra, MA Reg 1ANN57) and was on his way home in Watertown. When I asked for Kiflit's license, he told me that he only has a permit and has yet to take the test. He apologized repeatedly. Kiflit did not have any passengers in his vehicle. Kiflit said the roadway was so dark and he didn't see the parked Jeep. He claimed he was not speeding. There was minimal damage done to the drivers side rear bumper of the Jeep. The Nissan had extensive front end damage and was subsequently towed by Tody's Towing. Towed motor vehicle form filled out and filed, copy made for police prosecutor. Owner of the Jeep came out and took possession of the vehicle. Kiflit was taken to NWH for evaluation of his injuries. Kiflit's driver history shows that he had an surchargable accident recently 05/30/2020. Massachusetts Uniform Citation T2080927 was filled out and prepared to be mailed to Kiflit at his Watertown address. Henok Kiflit is being cited for: 90/10 - Operating without a License (Learner's Permit violation; Kiflit, operator, was not accompanied by any motorist)

MV1 was operating westbound on Watertown St (public way). MV2 was parked on Watertown St in front of 144 Bridge St (public way). MV1 operator struck and side swiped MV2. MV2 sustained extensive damage. MV1 continued westbound on Watertown St and struck a cross walk sign and Newton Fire Department box which both in front of Coletti-Magni Park, and both were destroyed. MV1 hit and hopped the curb of Watertown St in front of Coletti-Magni Park, drove onto the side walk, and hit the fence of Coletti-Magni Park and damaged much of it. MV1 then came at rest on top of the Coletti-Magni Park fence and sustained extensive damage. MV1 operator stated he was not injured at this time and sign a patient refusal with Newton Medics. MV2 was unoccupied. Both MV1 and MV2 were towed by Todys.

On Monday, August 10th 2020 at 0459 hours, I responded to the intersection of Brookline St at Vine St for a one vehicle accident into a City Of Newton Tree. The vehicle was a Black Honda Accord Mass Reg # 7DZ958. Upon arrival, Samuel Perez was outside the vehicle and no operation of the vehicle was witnessed. There was no drugs or alcohol inside the vehicle. The vehicle appeared to have been operated heading west on Vine St. The vehicle continued through the stop sign at Vine St straight into Brookline St where the vehicle collided into a City of Newton Tree. Pictures were taken and submitted to IT. The airbag was deployed. Perez complained of chest and head pain. Perez was taken by Ambulance to Newton Wellesley Hospital. There was extensive damage to the front of the vehicle. Tody's Towed the vehicle. A Towed Motor Vehicle Inventory form was completed. Perez was mailed Mass Uniform Citation #T1273442 in the amount of $ 105 for M.G.L Ch. 89/9 Failure to Stop at Stop Sign.

Operator of vehicle one, Agustine Velasco stated that on 08/10/2020 at 13:36 hours he was driving a city of Newton contacted Waste Management trash truck ( 2009 Atuo Xpedit ) northbound on Woodbine St. Woodbine St is a pubic way in the city of Newton. Velasco stated that as his vehicle passed a parked unoccupied northbound facing vehicle ( 2017 Rage Rover MA reg 5GD698 ) in front of 19 Woodbine St the mechanical arm that picks ups trash barrels was not stored in the fully closed position. Velasco stated that the mechanical arm's clamp struck the driver's side rear door and lower quarter panel of the Range Rover causing extensive damage. The truck's arm clamp was broken off in the crash. Owner of vehicle two Maura Brown stated her vehicle was parked unoccupied facing north in front of 19 Wood brine St prior to the crash. Neither vehicle was towed . Velasco stated that hen was not injured in the crash. I took photos of both vehicles and forward the disk to the NPD's IT Bureau for downloading. Based upon Velasco's statements to me about his truck's mechanical arm not being in the fully closed position, and my observations of the crash scene, I issued Velasco MA Citation T2015997 and cited him for a violation of city of Newton Ordnance 19/75, failure to use care while starting.

On 09/30/2020 at approximately 19:32hrs members of the Newton Police Patrol Bureau responded to a motor vehicle crash near the intersection of Washington St and Eddy St. After an initial investigation of the crash the Patrol Bureau secured the crash scene and called in the Newton Police Traffic Bureau to investigate. At approximately 19:45hrs Sgt. Devine, Ofc Wilson and I (Ofc Schlegel) from the NPD Traffic Bureau responded to the scene to investigate. Upon arrival to the scene I was met by Ofc Kane of the NPD Patrol Bureau. Ofc Kane stated that the crash was located in the driveway of 74 Eddy St and that it involved a Ford F350 pickup striking two parked cars. Ofc Kane stated that the operator identified as Kyle Butler had already been taken to Newton Wellesley Hospital by Cataldo EMS for injuries. . Upon approaching the crash scene I observed the following: The crash occurred at night time hours at approximately 19:32EST near the intersection of Washington St and Eddy St. Both Washington St and Eddy St are public ways in the City of Newton. The weather conditions at the time of the crash were a dark and clear with a temperature of approximately 60 degrees F. Washington St is a four lane undivided roadway (2 westbound travel lanes and 2 eastbound travel lanes). Washington St lighted roadway which has concrete light poles on both the westbound and eastbound sides. Washington St was under construction at the time and had a base coat of asphalt applied. At the time of the crash Washington St did not have any painted lane markings due to ongoing construction. Washington St is separated from the road sides on both the eastbound edge and the westbound edge with fixed granite curbing. The roadway also had several orange traffic barrels and traffic cones located edges of the eastbound and westbound lanes. Affixed to the light poles and the traffic barrels were temporary no parking signs for the ongoing road construction. At the time of the crash no road construction activities were taking place. No major debris or other obstructions were observed in the roadway. Washington St is a major roadway in the City of Newton and travels west to East across the entire City of Newton. This area of Washington St where the crash occurred is in between West Newton Square and Newtonville Square, this area has a posted 35 MPH speed limit. Both West Newton Square and Newtonville Square are densely populated with several businesses and residential areas. The section of Washington St where the crash occurred is also traveled frequently by pedestrian, bicycle and motor vehicle traffic. At the time the crash occurred 19:32hrs This area of Washington St is still a heavily travelled roadway. I observed Vehicle 1 described as a 2003 Ford F350 color white (MA Reg 5DT664) located in the driveway of 74 Eddy St. 74 Eddy St is a multi-family dwelling house located on the corner of Washington St and Eddy St. The driveway of 74 Eddy St is on the Washington St side. Vehicle was faced easterly across the driveway. I observed Vehicle 1to have extensive damage to the entire vehicle including but not limited to the enclosed cab area, cab roof and all windows, front end, rear end, along with both driver and passenger side. I also observed city of Newton owned concrete traffic light resting on top of the cab area of Vehicle 1. Upon further observation of Vehicle 1 it appears that both front tires and rims were flat, heavily damaged and had little to no tread left. Both front tires were worn down to the interior steel belt. I also observed that Vehicle 1 did not have a valid inspection sticker. Located to the immediate front of Vehicle 1 and resting on the front end was a white fence which had damage consistent with the impact of a motor vehicle. Immediately to the driver's side of Vehicle 1 was Vehicle 2 described as a 2007 gray Subaru Forester (MA Reg 45D610. Vehicle 2 was facing westerly across the driveway. Vehicle 2 had extensive damage to the driver's side front end, rear end, in addition to the entire driver and passenger sides. At this time Vehicle 2's drivers door was in contact with Vehicle 1's drivers door. Both drivers doors could not be opened.The damage to Vehicle 2 appeared to be consistent with impact from another motor vehicle. To immediate passenger side of the Vehicle 2 I observed Vehicle 3 described as a 2011 Toyota Rav4 color black (MA Reg 47XB99). Vehicle was facing in an easterly direction across the driveway. I observed the Vehicle 3 to have extensive damage to driver's side rear quarter panel, passenger side front quarter panel and front end. Also observed was damage to south east corner of the dwelling of 74 Eddy St. This area of the dwelling is located adjacent to the drive way. The damage appeared consistent with the impact caused by a motor vehicle. Also observed were large amounts of debris located in the area of the crash which included the sidewalk, yard and the driveway of 74 Eddy St. The debris included pieces of concrete which appeared to have been dislodged from the concrete light pole. A city of Newton no parking sign was lying in the yard of 74 Eddy St. This parking sign would have been attached to the concrete light pole due to ongoing road construction on Washington St. I also observed two tire marks on a curb stone located next the westbound lane of Washington St. These tire marks were in front of the residence of 74 Eddy St and were to the immediate left of a storm drain. The tire marks were consistent with being struck by a motor vehicle. In between the tire marks and just after the curb there was a hole in the ground with wires sticking out of it. This hole appears to be the location the concrete light pole was located prior to impact. Tire marks were located on Washington St traveling from the east bound lane easterly across the westbound lane and ending at the tire marks on the curb. The total length of the tire marks was approximately 190 feet. After conducting an on scene investigation it appears that Vehicle 1 from the start of the tire mark in eastbound travel lane of Washington St traveled 190 feet in the easterly direct across the westbound lane of Washington St and impacted the curb in front of 74 Eddy St. The tire marks had lateral striations almost the entire length indicating that the front tires were rolling and sliding with little to no braking prior to impact with the curb. After impact with the curb Vehicle 1 exited the roadway, struck a concrete light pole. Vehicle 1 then traveled over a sidewalk, across the yard of 74 Eddy St and into the driveway of 74 Eddy St. Located in the driveway of 74 Eddy St were Vehicle 2 and Vehicle 3. Vehicle 2 was parked facing toward the rear of 74 Eddy St. Vehicle 3 located to the passenger side of Vehicle 2 and was parked facing towards Washington St. When Vehicle 1 entered the driveway it struck Vehicle 2. The force of the impact of Vehicle 1 striking Vehicle 2 caused Vehicle 2 to strike Vehicle 3. The force of the impact of Vehicle 2 striking Vehicle 3 caused Vehicle 3 to strike the southeast corner of the dwelling located at 74 Elliot St. Vehicle 1 continued across the drive way and struck a white fence before coming to final rest. Vehicle traveled a approximately 55 feet from when it initially struck to the curb till final rest. No other injuries were reported on scene. All involved vehicles were towed from the scene by Tody's Towing. After the investigation the operator of Vehicle 1 Kyle Butler was issued Citations T2015882 and T2015883 for: Mgl 90/24/O Recklass Operation of a Motor Vehicle Mgl 89/4B/A Failure to Operate in the Right Lane Mgl 90/20 Failure to Submit a Motor Vehicle for Inspection Mgl 90/7 Defective Equipment (worn front tire treads) Mgl 90/13A Failure to wear a seatbelt An immediate threat form was also filed with the MA RMV. On Tuesday, October 6, 2020, I received a call at approximately 2158 hours from Sgt. Devine of the Newton Police Traffic Bureau. Sgt. Devine stated there was a motor vehicle crash with serious injury that had just occurred at the intersection of Crafts Street and Waltham Street in Newton. Sgt. Devine stated that one of the parties involved had suffered life threatening injuries and was transported to Brigham and Women's Hospital. I responded to the scene and conducted an investigation of the crash with the assistance of Sgt. Devine and Captain Boudreau. Upon arrival it was reported to me by officers on scene that a party operating a motorized scooter on Crafts Street (E) had been struck by a 2007 Chrysler Town and Country (MA: 5EE316). The Chrysler was attempting to make a left turn from Crafts Street (W) onto Waltham Street (S). The operator of the motorized scooter, identified as Nadhir Tchikou (DOB: 11/23/1993), had suffered serious injuries to his head and left leg area and was transported to Brigham and Women's Hospital. It was reported to me that Officers Conary, Boyle, and Donovan were the first to arrive at the crash and provided life-saving care to Mr. Tchikou prior to being transported to Brigham and Women's Hospital. Officer Donovan responded to Brigham and Women's to keep us updated on Mr. Tchikou's status. The update provided to me while on scene was that Mr. Tchikou was in critical condition and in surgery. Due to the seriousness of the crash and potential fatal injury, Sgt. Devine contacted the Massachusetts State Police to assist in the investigation. Sgt. Mahoney and Trooper Paul responded as part of the Massachusetts State Police Crash Reconstruction team and conducted a supplemental investigation of the crash (MSP report 2020-CAR-000285). The location where the crash took place is Crafts Street and Waltham Street. Crafts Street and Waltham Street are public ways maintained by the City of Newton. Crafts Street and Waltham Street are municipal roadways that support two way travel for motor vehicles and bicycles with an unprotected median in the middle of both roadways to separate the direction of travel. This intersection was recently re-paved within the last month and has newly painted street markings including crosswalks, dedicated bicycle lanes, and stops lines at the intersections. Crafts Street at this intersection features two travels lanes. I observed the road surface to be dry at the time of my arrival on scene. The north lane in the westbound direction of Crafts Street permits travel straight ahead and also permits motorists/bicyclist to take a right turn onto Harding Street. The south lane in the westbound direction of Crafts Street is a dedicated left turn only lane that coincides with a left turn only arrow on the traffic signal in the middle of the intersection. The opposite side of Crafts Street permits traffic traveling in the eastbound lane to travel straight or take a left turn onto Harding Street from is a single lane travel lane. The area is well lit featuring street lights bright enough to illuminate the intersection for motorist traveling through the area. The weather at the time of the crash appeared to be clear. In the intersection where the crash took place, I observed a large debris field comprised of parts from a 2020 Tao Tao motorized scooter (49CC) and a 2007 Chrysler Town and Country (MA: 5EE316). I observed the motorized scooter (VIN: L9NTCBAG1L1010287) resting on its side facing North in the northbound lane of Crafts Street in the intersection. The motorized scooter has significant damage to its front, right, and left side and was inoperable. The area in the immediate vicinity of the motorized scooter was saturated with gasoline. The rear lights and dash of the motorized scooter were illuminated while I was observing the crash scene. The headlights were not intact due to the impact from the crash and I was unable to determine if they were functioning. The motorized scooter was removed from the roadway by Tody's Towing. The intersection where the crash took place had numerous first responders present immediately after the crash rendering aid to Mr. Tchikou prior to his transport to the hospital. It was reported by officers on scene that some of the items and debris in the intersection had been moved from their initial place of rest after the crash took place. A black MMG helmet was observed resting on the ground outside the debris field from the crash before the westbound left turn only lane of Crafts Street. It is unknown if it was moved from its initial place of rest by a first responder. I observed the strap on the helmet that would be used to secure it to the operator to be in good operable condition. I did not observe it to be defected and the securing feature of the strap was in working order. I did not observe any blood, hair, or other apparent physical evidence inside the helmet that would be consistent with the head injury reported to the operator of the motorized scooter. I observed what appeared to be fresh markings on the front mouth/nose area of the helmet. I observed a white Iphone with a red phone case lying on the roadway a short distance away from the impact area. The Iphone was on and displaying travel directions from the Iphone application Waze. The Iphone and the helmet were seized pending the status of the operator of MV2. A property/evidence form was completed and the Iphone and helmet were secured in property locker number 9. I spoke with the operator of MV1, William Moran (S34946215). Moran stated he was operating his white 2007 Chrysler Town and Country (MA: 5EE316) on Crafts Street (N) as he approached Waltham Street. Moran stated he had a green light and activated his left blinker as he approached the intersection with the intention of taking a left turn onto Waltham Street. Moran stated as he proceeded to take his left turn he saw "orange" and was unable to avoid hitting the object in front of him. Moran stated the object came out of nowhere and he swerved to the left in an attempt to avoid the crash. Moran stated after the collision he accelerated away from the impact area and came to a controlled stop on Waltham Street (S) just past the intersection. Moran stated he exited his vehicle and looked back at the intersection he had just travelled through. Moran stated as he began walking towards the intersection he observed a party laying on the roadway in the intersection. Moran stated the party was not wearing a helmet at this time. Moran also added that he believes the moped involved in the crash did not have its headlights on prior to impact. Moran stayed on scene and cooperated fully with officers during the investigation. A query of Moran's license and registration status through Newton Police Dispatched showed they are both active. I asked Moran if he was using a cell phone, GPS, or any other type of hand held device prior to the crash. Moran stated he was not. I asked Moran if he was distracted by anything either inside the vehicle or something on the roadway prior to the crash. Moran stated he was not. I asked Moran if he was currently impaired in any way that would impede his ability to operate a vehicle safely. Moran stated he was not. I asked Moran if he takes any medications or has a medical condition that would impede his ability to operate a vehicle safely. Moran stated he does not. Moran reported no injuries as a result of the crash. I observed Moran's vehicle at its controlled rest location. The vehicle was parked on Waltham Street (S) in the southbound lane just past the intersection of Crafts Street. The 2007 Chrysler Town and Country (MA: 5EE316) had substantial damage to the front passenger side bumper, hood, fender, windshield, and passenger door area. There was physical evidence of what appeared to be hair, blood, and tissue presumed to be from the operator of MV2 present in the cracked windshield, and on the passenger side door/mirror area. I observed a blue Commonwealth of Massachusetts "R" inspection sticker with a date of 05/28/2020 affixed to the windshield. Moran stated the rejection sticker was a result of an emissions failure. The inspection sticker expires in 2021. I asked Moran if there were any other mechanical issues with the vehicle. Moran stated there were no issues to his knowledge. I observed no objects present inside the vehicle during my investigation that would have potentially impeded the operators ability to see clearly through the windshield. I observed glass debris from the windshield inside the passenger and driver area of the vehicle. There was no air bag deployment inside the vehicle. Subsequent to my investigation, the vehicle was seized pending the status of the operator of MV2. The vehicle was towed from the scene by Tody's towing and secured in the Newton Police Department garage. The vehicle was escorted to the garage by Officer Donovan. A towed motor vehicle form was completed by Officer Donovan and the key for the vehicle was secured in property locker number 9. A property/evidence form was completed. Subsequent to my investigation Moran's black Alcatel cell phone was seized pending the status of the operator of MV2. A property/evidence form was completed and the phone was secured in property locker number 9. Officer Conary stated to me that prior to my arrival on scene she interviewed a witness to the crash and provided me with the following information. Joshua Hamel was operating his vehicle (W) in the right lane of Crafts Street towards Waltham Street. Hamel stated that MV1 (MA: 5EE136) was in the left turn lane on Crafts Street (W) towards Waltham. Hamel stated the scooter was headed eastbound on Crafts Street from Waltham. Hamel stated MV1 began to take a left turn and there was a head on collision with the scooter. Hamel stated he believed the traffic light at the time of the crash was green. On Friday, October 9, 2020, I spoke with Hamel via telephone. Hamel corroberated the statement he gave to Officer Conary. I asked Hamel if he believed the headlights on the motorized scooter were activated at the time of the crash. Hamel stated he believes the headlights on the motorized scooter were on as it approached the intersection but was not postive. On Wednesday, October 7, 2020, I spoke with Mr. Tchikou in his ICU room at Brigham and Women's Hospital. Tchikou's wife, Yami Carrajza, was present in the room while I spoke with him. Tchikou stated he had a significant head injury, injuries to his nasal cavity, his back, and a significant injury to his left leg. I asked Tchikou about his recollection of the crash. Tchikou stated he had just left his friend's home on Hazelhurt Avenue in Newton and was traveling home to Brighton to bring his wife dinner. Tchikou stated he was traveling Eastbound on Crafts Street as he approached the intersection of Crafts Street and Waltham Street. Tchikou stated he had a green light and as he entered the intersection a white vehicle took a turn from the opposite side of the road in front of him. Tchikou stated he does not remember anything after that. I asked Tchikou if he was wearing a helmet at the time of the crash. Tchikou stated he was. I explained to Tchikou that I recovered a white Iphone with a red case at the scene of the crash. Tchikou identified the Iphone as his. I explained that the phone was displaying travel directions when I found it and I asked Tchikou if he was distracted by his phone prior to the crash. Tchikou stated he was not distracted by the phone and the phone was in a holder on the motorized scooter. I asked Tchikou if he remembers if his headlights were activated on his motorized scooter at the time of the crash. Tchikou stated they were on. I asked Tchikou if he could produce paperwork for the motorized scooter to prove he was the owner. Tchikou stated he had recently purchased the scooter and the paperwork for it was at his residence. Tchkou's wife e-mailed me a picture of the Certificate of Origin for the motorized scooter. I asked Tchikou if he could produce a license or permit that would allow him to operate the motorized scooter. Tchikou stated he did not have a Massachusetts license or permit, but had an Algerian license at this residence. I was able to observe a picture of the license on his wife's cell phone. The written text on the license was in Arabic. Tchikou did not have a translation of the license in English and did not possess an International Driver's Permit. As a result of my investigation, Mr. William Moran while operating MV1 (MA: 5EE316) entered the travel lane of MV2 (Tchikou) as he was making a left turn onto Waltham Street from Crafts Street. MV2 had the right of way and MV1 failed to yield to MV2's right of way causing the collision to take place. Moran was advised of my investigation and issued Massachusetts Uniform Citation T0115914 via mail by request for Chapter 90, Section 8 (Fail To Yield To The Right Of Way At An Intersection.) Mr. Nadhir Tchikou operated his motor scooter at the time of the crash without a valid license/permit. Tchikou is also in violation of operation of a motor scooter after dark (prohibited). The crash occurred at 2147 hours. Tchikou will be issued Massachusetts Uniform Citation T0115915 in hand at Brigham and Women's Hospital for Chapter 90, Section 1E (Unlicensed Operation Of A Motor Scooter) and Chapter 90, Section 1E (Operation Of A Motor Scooter After Sunset - Hours Restriction.)

Appears that MV#1 was traveling north on Seminary Ave when it veered to the left and struck the curb and then a light pole on the side of the roadway. MV was parked in a safe spot on Lasell property as it had only minor damage.

On November 1st, 2020 at approximately 14:02 hours while assigned to N491 I responded along with Newton Fire and Medic One to the intersection of Pearl St @ Jackson Rd for a MV crash with minor injuries. On my arrival I first observed in the intersection a green colored Volkswagon with Vermont tag,HPK270 with extensive damage. The other involved vehicle a white Jeep Wrangler, bearing Ma tag, 8WK846 was located on the property of the Lincoln Elliot School, 191 Pearl St, also with extensive damage. The operator of the green Volkswagon was identified as Viorel Marin. He stated he was going E/B on Pearl St and was approaching the intersection of Jackson Rd when the light had turned green. As he was entered the intersection vehicle #2 came from his right side(N/B) on Jackson Rd at, according to him, a high rate of speed causing his vehicle to collide with the Jeep. He doesn't remember much after that. I observed a small cut to his right knee and later he showed me a small abrasion to his forehead. All his air bags had deployed on contact with vehicle #2 . I located the operator of the Jeep who was sitting on the ground being assisted by an unidentified women(not a passenger). She appeared shaken but had no visible signs of any serious injury from the crash. She identified herself as Tara Feeley. According to Ms Feeley she had been travelling N/B on Jackson Rd and was attempting to continue straight across the intersection of Pearl St when she was struck by vehicle #1 in the intersection. The impact caused her vehicle to flip over unknown amount of times finally landing on the property to the Lincoln Eliot School. Based on operator #2's statements, my observations of the scene, damage to her MV, it appeared her vehicle after contact with vehicle #1, flipped over N/B across the intersection first hitting the traffic light on the northeast corner, additionally making contact with the utility pole just east of the traffic light before finally landing right side up on the property of the Lincoln Eliot School. She reported climbing out her passenger side window when her vehicle finally stopped. She said,"she was lucky she had her seat belt on". I agreed. Ms Feeley also reported that she was sure that her light was green as she entered the intersection. I attempted to locate any possible witnesses from the crowd that were gathered near the crash site. There were two that heard the crash but none that actually saw what happened. I couldn't locate any cameras near the intersection. Both vehicles were towed from the scene due to the extensive damage/both totaled. Both operators had minor injuries and both declined to be transported to the hospital. They signed patient refusals. Officer Danny Anderson later took several pictures of the crash scene and the all the property damage. Both utility companies were notified. The camera disc was placed in the technology bureau mailbox.

Operator states that she was driving westbound on River St when she felt her vehicle hit something in the road. She did not see anything, but completely lost control of MA reg 1LFH86. The vehicle ended up hitting a utility pole and sustained heavy damage/airbag deployment. Upon inspecting the scene, a rotor was located on River St near Auburndale Ave. It appears that the vehicle's right front balljoint snapped and caused severe mechanical failure, in turn causing the operator to lose control. Medics documented a patient refusal and Tody's towed the vehicle to their lot. Eversource was notified to check the integrity of pole #25 in front of 169 River St.

Operator of MV1 states he was parked at the meter spot in front of 1175 Walnut Street when he went to merge onto Walnut Street southbound. MV2 struck his front bumper with the right side of her vehicle. Operator of MV1 states he spoke with the operator of MV2 who stated to him she didn't see him merging onto Walnut Street because she was on her cell phone. MV1 sustained damage to the front bumper. Operator of MV2 states she was traveling southbound on Walnut Street and struck MV1 when he was merging into traffic. I asked the operator of MV2 if she was on her cell phone, which she first denied, but admitted to having the cell phone in her hand. MV2 sustained extensive damage to the right side of the vehicle and needed to be towed, which was removed from the roadway by Tody's towing. Massachusetts uniform citation #T2012863 was issued to the operator of MV2 for MGL Ch. 90 Sec. 10 Operating MV w/o a license and MGL Ch. 90 Sec. 13B Use of Electronic Device. No injuries were reported on scene.

On 12/05/2020 at approximately 22:00 hours I was called in to respond to a single vehicle crash with serious injuries in the area of 291 Nahanton St. Nahanton St is a pubic way in the City of Newton. Upon my arrival at the crash scene with Sgt Devine I was briefed by Officers Daly and Anderson, ( see Ofc Daly's report for further information CAD # 20054728 in regard to changes take out against the operator of the truck ) I made the following observations of the crash scene, Nahanton St is a public way in the City of Newton that is a two way roadway divided by an unprotected merdium ( double yellow line ) that runs east to west. The roadway surface was wet due snow and ice. The roadway was plowed and the snow was pushed to the shoulder, althought wet the roadway was icy. There was approximately 2 inches of snow on the shoulders of the roadway. The road was light with overhead head street lights. The posted speed limit is 25 MPH. I observed a 2013 Chevrolet Silverado pick up truck that had major front end damage. The Chevrolet was at an angle with it's front end facing west bound and resting on the grass shoulder and into a wooded area. The Chevrolet rear was sticking out into the shoulder of the east bound lane of Nahanton St. The passenger side front and rear doors of the Chevrolet had been removed by the fire department to extricate the driver and passenger. The driver and passenger side air bags were deployed . The front passenger side windshield of the Chevrolet was completely missing and the driver's side had completely shattered. The front tires, hood, and engine had been compacted on it's self. The front and rear driver's side doors had been mechanically open by the fire department. The speedometer needle had stopped at 54 MPH. The Chevrolet left the roadway and left tire marks in the snow in the eastbound bike lane of Nahanton St. At a 106.4 feet from the beginning of the tire marks the first point of impact was with two tree stumps that were 3.5 feet from the roadway, four feet tall and 3.5 feet in diameter. The tire marks continue in the east bound bike lane and exit the roadway and onto a dirt shoulder at 136.1 feet from the beginning of the tire marks. The truck continued to the right of a utility pole and then it's passenger side made contact with a tree at 170.8 feet from the begin of the tire marks. The tree was 12 feet from the bike lane approximately 5 feet in diameter. The impact with the tree caused the truck to turn so that the rear was facing north east on the dirt shoulder and bike lane. The final point of rest of the rear passenger side tire was 200 feet from the beginning of the tire marks. The truck's rear diver side tire final point of rest was 209 feet from the beginning of the tire marks. The front driver's side tire was compacted into the trucks engine and the passenger side front tire was located behind the tree approximately 20 feet from the roadway. Sgt Devine took photos of the crash area and of the truck and the disk was download by the NPD's IT Bureau. The truck was towed from the crash scene by Todys's Towing to their lot for storage, I fill out a towed motor vehicle form. There were no reported witness who observed the crash. The crash was reported by an unknown passerby. On 12/07/2020 Ofc Gaudet faxed a request for preservation of medical records, blood and urine samples of Schneider to Beth Israel Deconess Medical Center.

On February 1st, 2021 at approximately 07:53 hours while assigned to N491 I responded to MV crash in front of #41 Bridge St. On my arrival I observed a large white box style truck crashed into a gray SUV both vehicles with extensive damage blocking the street completely. I first identified the operator of the box truck, Maurizio Didonna. The truck belonged to Russo's & Sons out of Pleasant St Watertown Ma. Mr Didonna reported driving S/B on Bridge St when he temporarily got distracted before crashing into a parked vehicle in front of #41 Bridge St. The owner of the parked vehicle was not on scene but a male construction worker who was building a new house at #41 Bridge St identified the vehicles owner as Jose Ramirez. He was able to make contact with him and informed him about the accident. Both vehicles were towed from the scene. One by Todys. Russo's & Sons truck was towed by Pro Tech out of Watertown. Mr Didonna had a minor injury but refused any medical treatment. Note:The parked SUV had a lot of construction equipment in the rear but was removed by the construction workers from the job site at #41 Bridge St. The parked car was affiliated with the construction site. The vehicles owner was notified to where his vehicle was being towed to. .

On 2/23/21 at 1900 hrs, Newton Police were dispatched to the intersection of; Lowell Ave. at Otis St. for a motor vehicle collision involving two vehicles. The operator of MV#1 stated they were attempting to turn eastbound onto Otis St. from Lowell Ave. when MV#2 began to pass MV#1 on the left. As MV#2 was passing MV#1, MV#2's passenger side collided with MV#1's front snow plow. Extensive damage was observed to MV#2's passenger side rear and front door, while also puncturing MV#2's passenger side rear tire. No damage was observed to MV#1. The operator of MV#2 stated he attempted to pass MV#1 on the left, as he believed MV#1 was going to turn right onto Otis St. No injuries were reported and MV#2 was towed from the scene by Tody's Towing Service, while MV#1 was driven from the scene.

On Monday May 31, 2021 at approximately 2152 hours I came upon a single MVA on Waltham St (a public way ). MV1 operator, who has an active license from Guatemala, stated he was operating southbound on Waltham St at Llewellyn Rd (public way). MV1 operator said after he passed Llewellyn Rd he may have been on his phone. MV1 operator said he struck the curb of the southbound side of Waltham St, and then struck the public tree. MV1's airbags were deployed. MV1 operator stated he was injured, and he was visibly bleeding from the nose. Newton medics transported MV1 operator to Newton-Wellesley Hospital. MV1 sustained extensive and total front end damage. MV1 was towed by Todys. The tree struck by MV1 did not appear to have sustained major damage and is not a safety hazard at this time.

MV1 operator stated she was operating eastbound on Washington St (a public way) driving the speed limit. MV1 operator stated she observed MV2 on the westbound side of Washington St attempt to turn left into Starbucks (2322 Washington St). MV1 operator stated by the time she saw MV2 cut across in front of her and into her lane, it was too late and MV1 struck the passenger side of MV2. MV2 operator stated the same story. MV2 operator said she was also going the speed limit on Washington St westbound and attempted to turn left into Starbucks but misjudged MV1's speed, causing the MVA. MV1 sustained extensive total front end damage and was towed by Tody's. MV1 operator requested an evaluation by Newton Medics and was transported to Newton-Wellesley Hospital. On Thursday 7/15/21 at approximately 0900hrs I was dispatched to 12 Rockland Street for the report of a motor vehicle with heavy damage resting in the middle of the street. Upon arrival I observed a brown Nissan Altima unoccupied not running in the middle of street. Heavy front end damage on the passenger side along with two flat tires on the passenger side. There was no operator on scene but the car was registered to 12 Rockland Street. It should be noted that Rockland Street is a public way located in the City of Newton. I spoke to the reporting party Barry Cocuzzo who lives at 18 Rockland Street. Mr. Cocuzzo stated he witnessed William Power operating the Nissan and turn onto Rockland Street. Mr. Cocuzzo observed Power drive into his driveway with the two flat tires and heavy front end damage. When Power exited the vehicle Cocuzzo yelled over to him but stated Power's was unable to communicate clearly to him. Cocuzzo observed Power's walk into his residence at 12 Rockland and then bereaved the vehicle slowly roll back out of the driveway. Medic 3 and Tody's responded to the scene. Due to the heavy damage and two flat tires Tody towed the vehicle. A towed motor vehicle inventory report was filled out. Medic 3 was called to the scene to evaluate the health of Power's. Officer Tocci and myself located Power's in the residence passed out on the couch. Empty alcohol cans were all around Power's. After several attempts to wake up Power's he finally came too. Power's speech was severely slurred, he could barely keep his eyes upon and he lacked the ability to stand up without assistance. The Medics transported Power's to Newton Wellesley. Power's is being cited (Ma Citation 069847AB and 06850AB) for 90/10/A Unlicensed Operation, 90/24/E Negligent Operation of MV, 90/24/C Leave Scene of Property Damage, 90/24/J OUI Liquor and 90/24/P Use MV Without Authority. 5 photos were taken of the motor vehicle. It should be noted that the original location of the crash and the object that was struck are unknown to us at this time. Dispatch was also able to confirm that Power's did not have an active license at this time. Power's mother Bernice Fougere who is the owner of the vehicle stated to me that Power's did not have permission to use her vehicle.

MV1 was heading Westbound on Newtonville Ave through the intersection of Harvard St when he collided with MV2. MV2 was heading Northbound on Harvard St through the same intersection when it collided with MV1. Both operators stated that they had the green light to proceed through the intersection. MV1 collided with MV2 and then rolled over to its top, laying to rest on Newtonville Ave up against a residential fence belonging to #355 Newtonville Ave. MV2 obtained extensive damage to the front and all airbags deployed. Both operators signed refusals with the medics. MV1 operator, Everton Roberto-Rosa was issued in hand MA Citation #T2080934 for 90/10 Unlicensed Operation. Both vehicles were totalled and towed by Tody's. Two witnesses saw the accident. The first witness, Jessica Foz, stated "wow, look at him, he is not stopping" and stated that MV1 was driving too fast and ran a red light. Another witness was Georgia and Harry Triantafilles who both stated that MV1 had the green light.

At approximately 1640HRs on Thursday, August 12, 2021, I came across a motor vehicle crash at The intersection of Grant ave. and Commonwealth Ave.'s Carriage Lane. MV 1 (MA Pass: 882WWK) was facing SB on Grant ave., in the intersection. I observed it to have extensive front end damage (bumper, hood, driver's side head light and a pushed in radiator core support) and it was leaking fluids. MV 2 (MA Pass:8RS517) was facing WB in the Carriage Lane and displayed damage to its front passenger's side ( hood, head light, bumper and fender). Upon speaking to the operator of MV1, they stated that they were traveling straight (SB) on Grant Ave. and had a green traffic light. As they approached the intersection, MV 2 made a left hand turn and struck MV 1. Upon speaking to the operator of MV 2, they stated they were traveling on Grant Ave. (NB) and attempted to make a left hand turn onto the Carriage Lane (WB). It was at this time they struck MV1 because they did not see them. Upon speaking to witness 1, they stated they were traveling behind MV1 and that the traffic light was indeed green. They then observed MV 2 traveling at a high rate of speed, as if they were attempting to "cut off" MV1. When MV 2 attempted to make a left hand turn, they impacted into MV1. All parties involved stated that they were not injured and did not wish to been seen by EMS. Newton Fire arrived on scene and cleaned up the fluids. MV1 was removed by Todys service and MV2 was able to drive away under its own power. I gave the operator of MV 1 a ride to NPD HQ to await their ride and then cleared.

Wade stated a serious motor vehicle crash occurred on Washington Street in Newton and it involved a City of Newton marked police cruiser (N494). Sgt. Wade stated Officer Katelyn Pohlman and the operator of the other vehicle involved were transported to the hospital with potential serious injuries. I was requested by Sgt. Wade to respond to the scene from my residence and investigate the crash further. On arrival at the scene, I observed the two vehicles involved in the crash to be a 2015 Ford Explorer (MA: 586RT7) and Newton Police marked unit N494 (MA MVN: MP494B). These vehicles were observed disabled on the roadway in front of 612 Washington Street, Newton. There was a large debris field comprised of various parts from both vehicles on the roadway and in the parking lot of 612 Washington Street. The road surface at the time of my arrival was wet and there was light rain falling. Sgt. Wade reported that at the time of the crash the road was completely dry and it had not started to rain yet. Washington Street is a public way maintained by the City of Newton. The portion of Washington Street where the crash took place supports two- way travel, Eastbound and Westbound. There are two travel lanes in both directions. Travel direction is divided by a double yellow line in the middle of the roadway. The area is well lit and features numerous light poles illuminating the roadway. The closest intersection to this location is Crafts Street just west of the crash location. MV1 (MA: 586RT7) had substantial front-end damage, especially to its front passenger side and was inoperable. I observed this vehicle at its final rest on the roadway with its front end facing Northbound on Washington Street just over the double yellow line in the middle of the roadway. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV1 was removed from the roadway by Tody's Towing. MV2 (MA MVN: MP494B) had substantial front end damage and was inoperable. The impact from the collision caused the vehicle to spin uncontrolled and the final rest, with the front end of the vehicle facing the front entrance to the 7-11 (S). The vehicle's final rest was on the sidewalk in front of 612 Washington Street. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV2 was removed from the roadway by Tody's Towing. The Officer involved in the crash is Officer Katelyn Pohlman. Officer Pohlman stated she was operating Newton Police marked unit N494 on Washington Street (E) past Crafts Street. Officer Pohlman stated as she was operating her vehicle past 612 Washington Street (7-11), a vehicle in the westbound lane of Washington Street abruptly crossed the double yellow line in the roadway and entered her travel lane head-on. Officer Pohlman stated she did not have an opportunity to react to the vehicle in front of her and the front passenger side of MV1 crashed into the front end of her vehicle (MV2). Officer Pohlman sustained injuries to her left forehead area and both of her arms. Officer Pohlman was later transported to Brigham and Women's Hospital for evaluation and was released a short time later. It was reported to me by numerous Officers on scene that Officer Pohlman exited her cruiser after the crash and rather than tend to her own injuries, she focused her efforts on rendering aid to the operator of the other vehicle involved. Officer Pohlman found the operator of the other vehicle lying on the floor of the front passenger seat under the dash. Officer Pohlman rendered aid to the operator while waiting for Newton Medics to arrive. Officer Pohlman was also assisted in rendering aid to the other operator by Officer Conary and Officer Larson. Officer Larson reported that while interacting with the operator of MV1, the operator repeated numerous times, "Is she ok? Is she ok?". Officer Larson also stated the operator said to him "I feel bad because I was texting on my phone". Officer Larson stated the operator of MV1 works for the Department of Corrections Special Operations Unit and has Department issues equipment inside the vehicle. The following items were placed into Support Services, locker 9 for safekeeping by Officer Larson: 1 Gray Pelican Gun Case, 1 Black Gun Belt, 1 Taser, 1 Can OC Spray, 15 (50) Round Cases Of Ammunition. The operator of MV1 (MA: 586RT7) was identified as Mr. David Cafarelli (S16965940). Mr. Cafarelli was transported to Newton Wellesley Hospital for a potential head injury before my arrival. I spoke with Mr. Cafarelli at Newton Wellesley Hospital in the Emergency Room with Sgt. Wade. Mr. Cafarelli's mother and father were present when we spoke with him. Mr. Cafarelli stated he was out with friends before the crash and was driving to the 7-11 on Washington Street. Mr. Cafarelli admitted that before the crash, he had his cell phone in his hand while he was operating his vehicle. Mr. Cafarelli stated he does not remember if he was texting or changing the music on his phone at the time of the crash. Mr. Cafarelli stated he looked up and was unable to avoid the vehicle (N494) directly in front of him. Mr. Cafarelli stated he also does not believe he applied his brakes before the collision. When we spoke with Mr. Cafarelli in the Emergency Room, he was still waiting for test results to determine the severity of his injuries. On Monday, August 23, 2021, Sgt. Wade responded to 612 Washington Street, Newton to retrieve surveillance footage from the 7-11 of the crash. 7-11 stated their outside surveillance system was not operating correctly, but they were able to capture some of the incident from a camera inside the store. I observed this footage. MV1 is observed traveling on Washington Street (W) in the westbound travel lane in front of the storefront. MV1's driver side is then observed traveling over the double yellow line in the middle of the roadway and into the eastbound lane of Washington Street. A head on collision ensues and MV1 can be seen rotating counterclockwise in the roadway backwards to its final rest location on Washington Street. MV2 is then observed at its final rest position on the sidewalk facing southbound in front of the 7-11 storefront. After completing my investigation, I have surmised the following. Mr. Cafarelli was operating his 2015 Ford Explorer (MA: 586RT7) in the area of 612 Washington Street, Newton. Mr. Cafarelli was holding his cell phone in his hand while operating his vehicle. Mr. Cafarelli became distracted while he was operating his vehicle due to his attention being focused on his cell phone. Mr. Cafarelli traveled over the double yellow line from the westbound lane of Washington Street and entered the eastbound lane of Washington Street. Newton Police marked unit N494 was traveling on Washington Street (E) in the area of 612 Washington Street at this time. Mr. Cafarelli's vehicle crashed into the front end of Newton Police marked unit N494. Mr. Cafarelli was not wearing a seatbelt and as a result of the impact from the crash, he was ejected from the driver seat and his final rest inside the vehicle was in the flood of the front passenger seat. As a result of my investigation, Mr. Cafarelli was issued Massachusetts Uniform Citation for Chapter 90, Section 13B (Hands-Free Violation), Chapter 89, Section 4a (Marked Lanes Violation), and Chapter 90, Section 13a (Seat Belt Violation). Photos from the crash scene and the surveillance media from 7-11 capturing the crash were submitted to the IT Bureau.

The pedestrian, identified as Yuanhua Wei stated that she was walking west on Derby St and attempting to cross Parmenter Rd in the cross walk when she was struck by MV#1. The operator of MV#1 stated that she was traveling east on Derby St and began turning left onto Parmenter Rd because in her words, she didn't observe the pedestrian. The operator stated as she turned through the intersection, she observed the front of her MV strike the pedestrian, the pedestrian dropped to the ground and the MV#1 pulled over to the right side of Parmenter Rd. When asked, the operator stated she were not distracted while operating her MV. When I arrived Mrs. Wei was sitting in the cross walk directly next to the north west corner of that intersection. When asked about her injuries, she stated her hip hurt, she showed me an abrasion on her right elbow/forearm and her lower body was sore. I also observed one of Mrs. Wei's sneakers was on the ground next to her. Mrs. Wei was sitting the entire tire time, did not appear to have any serious injuries and attempted to get up shortly after I arrived, but advised her to remain sitting. Paramedics evaluated Mrs. Wei for minor injuries at the scene and she chose not to be transported to a local hospital for her injuries. I did not observe any damage to MV#1, which the operator confirmed. I informed both parties that I was filing a crash report with charges pending.

On 9/4/2021 at approx 1233hrs while assigned to 497 I responded to the intersection of Grove St and the 128 N Off ramp for a report of a two car crash with possible injuries. Upon arrival I observed Ma Reg 1FHT59 a gray Toyota Rav4 with extensive front driver side damage with air bags deployed and Ma Reg 6NW414 a black Cadillac SRX with extensive passenger side damage both up on the NW curb of the 128 N On Ramp. Both operators and a child passenger were out of the car. I spoke with the owner of the Rav 4 , Wenmen Pan who stated he was travelling W B on Grove St with his 5 year old son Calvin Pan when the operator of the Cadillac drove off the 128 N of ramp without stopping striking him on the driver side and pushing him up onto the curb. I spoke with the operator of the Cadillac, Rosemary Twumasi who related she did not see the stop sign. Both vehicles towed by Todys, refusals signed with Fallon Ambulance.

On Tuesday, September 21, 2021, I received a call from Sgt. Mike Wade of the Newton Police Traffic Bureau. Sgt. Wade stated a serious MVA/Bicycle crash had just occurred at the intersection of Cherry Street and Derby Street in Newton and the bicyclist was a 10-year-old boy with a possible head injury. Sgt. Wade stated the Traffic Bureau was requested to respond by the Shift Commander and asked if I could respond to investigate the crash. On arrival, Officers reported to me that the cyclist involved, Benjamin Lavoie, was transported to Children's Hospital in Boston with a possible head injury. It was reported to them that Benjamin was traveling Southbound on Cherry Street towards Derby Street on his bicycle. Benjamin entered the intersection of Derby Street and crashed into the front passenger side area of a passing motorist that had a green light. Officers on the scene also reported that the motor vehicle and the bicycle involved had been moved from their original rest location. Cherry Street is a public roadway maintained by the City of Newton. It supports two way travel (N and S) from Washington Street (S) to Waltham Street (N). The travel lanes are divided by an unprotected double yellow line median. The posted speed limit for this street is 25 miles per hour. Derby Street is a public roadway maintained by the City of Newton. It supports two way travel (E and W) from Waltham Street (E) to Lexington Street (W). The travel lanes are divided by an unprotected double yellow line median. The posted speed limit for this street is 25 miles per hour. The weather at the time of the crash was clear and sunny. The road surface in the immediate area of the crash was dry. I spoke with the operator of MV1, Ms. Erica Pusateri (S61409866). Ms. Pusateri stated he was operating her 2021 Kia Seltos (MA: 8HC743) Westbound on Derby Street towards Cherry Street. Ms. Pusateri stated that she had a green light at the intersection and proceeded to cross Cherry Street to continue onto Derby Street. Ms. Pusateri stated halfway through the intersection she heard a noise and saw a bicyclist hit the front passenger side door area of her vehicle. The collision caused the glass window from the front driver-side door to shatter. Ms. Pusateri's vehicle controlled her vehicle to a stop and she observed a young boy lying on the roadway in the intersection behind her. Ms. Pusateri exited her vehicle, ran over to the injured bicyclist, and rendered aid to him by stabilizing his neck. Ms. Pusateri remained on scene and cooperated fully with Officers. I asked Ms. Pusateri if she was distracted at all while operating her vehicle prior to the collison. Ms. Pusateri stated she was not. I asked Ms. Pusateri if she could recall where in the intersection the bicyclist crashed into her vehicle. Ms. Pusateri stated the bicyclist crashed into her vehicle in the Northwest side of Derby Street just outside of the marked crosswalk in the intersection. Ms. Pusateri stated she knows it was in the street in the intersection because she observed a woman with a stroller on the sidewalk on the corner of Cherry Street at Derby Street on the Franklin Elementary School side and she had already travelled through the intersection passed the East side of Cherry Street. Due to the unknown injury status of the bicyclist, Ms. Pusateri's vehicle was towed to the Newton Police Station and secured in the garage area as evidence. Ms. Pusateri reported a small laceration on her right hand with no other injuries. I observed damage to the front passenger side fender/door area. There was no airbag deployment as a result of the crash. I observed glass through out the front passenger side compartment area of the vehicle from the window breaking. I contacted Benjamin' s parents, Cheryl and John, to check on the status of their son. Cheryl stated Benjamin did not suffer any life-threatening injuries and appears to have bumps, bruises, and a cut on his head. I asked if I could speak with Benjamin for his account of what happened at the crash. Benjamin stated he was riding his bicycle on Cherry Street (Southbound) on the sidewalk on the East side of Cherry Street. Benjamin stated he left the sidewalk and entered the intersection to cross Derby Street and continue on Cherry Street. Benjamin acknowledged that at this time Cherry Street had a red light and Derby Street had a green light. Benjamin stated he then crashed into a passing car in the roadway. Benjamin was wearing a helmet at the time of the crash. The helmet had significant damage to it as a result of the collision. I told Benjamin his bicycle was being held at the Newton Police Department for safekeeping and we will arrange to get it back to him. Benjamin is a student at the Franklin Elementary School. I make contact with a witness to the crash, Mr. Benjamin Crosby. Mr. Crosby stated he did not witness the actual crash, just the aftermath. I was able to reviewthe 9-1-1 call Mr. Crosby made regarding the crash in Newton Disptach. Mr. Crosby stated a boy on a bicycle was in a crash with a motor vehicle. Mr. Crosby is heard saying "he went through the light" to the Dispatcher taking the call. After completing my investigation, I have summized the following. The bicyclist (Benjamin Lavoie) traveled down Cherry Street (S) and went through a red light to cross the intersection at Derby Street (W). As a result of traveling through the red light and entering the intersection, he crashed into the front passenger side door area of a passing motorist that had a green light (Erica Pusateri). No citation will be issued as a result of this crash.

MV2 operator stated he had exited Rt.95 northbound at the Grove St exit. MV2 operator said he was attempting to drive straight on to Recreation Rd/Rt.95 northbound on-ramp. MV2 operator said he stopped at the stop sign at Grove St and looked both ways. MV2 operator stated he did observe MV1 driving southbound on Grove St at a reasonable speed. However, MV2 operator said he thought MV1 had stopped to let him go. MV2 operator said as he attempted to drive straight onto Recreation Road/Rt.95 northbound on-ramp, he struck the driver side of MV1. MV2 operator reported no injuries at this time. MV2 sustained extensive front end damage, and the driving wheel airbag was deployed. MV2 was towed by Todys.

On Wednesday October 27, 2021 at approximately 0807 hours I responded to Cherry St at Derby St (both public ways) for a two car motor vehicle accident. Be advised at this time the weather was very rainy and cloudy, and the roads were covered in leafs and were wet. MV1 was operating on Cherry St northbound by the Derby St intersection behind MV2. MV1 operator stated he stopped as the intersection traffic light was turning yellow. MV1 operator stated he stopped with at least one car length between his MV at the intersection. MV1 operator stated MV2 continued halfway through the intersection but stopped as the light was turning yellow. MV1 operator stated after MV2 stopped in the middle of the intersection, it reversed backwards and struck his MV. MV1 sustained extensive front end damage and was towed by Todys. MV1 operator reported no injuries at this time. MV2 operator stated she was on Cherry St northbound at the Derby St intersection. MV2 operator stated when the intersection traffic lights turned yellow she stopped in the middle of the intersection, and put her MV in reverse. MV2 operator stated because of the slippery and wet road conditions, her MV reversed too quickly and slid, striking MV1 which was already stopped behind her. Upon arrival, vehicle 1 was rolled over and facing Eastbound in the Westbound lane. There was extensive damage all over the vehicle and it was leaking fluids. It appears that vehicle 1 was traveling Westbound on Commonwealth Ave and struck a tree and then crashed into the guard rail causing it to roll over. There was damage to a city tree and damage to the guard rail. Operator 1 stated (through translation from Officer Acuna), he was traveling Eastbound on Commonwealth Ave at approximately 25 MPH when he was looking at his GPS and his brakes gave out causing him to crash. When asked again, his story changed and he was not sure what direction he was traveling in. Newton Fire was able to retrieve operator 1's cell phone in the vehicle which was on a loop playing videos on instagram. Operator 1 was transported to Beth Israel Hospital by Fallon Ambulance. Tody's towed vehicle 1 and pictures were taken of the damage to city property and will be submitted to the IT Bureau to be attached to this report. Operator 1 was served in hand, Massachusetts Uniform Citation: T1445611 for the following: Ch. 89 s. 4A - Marked Lanes Violation Ch. 90 s.13B- Electronic Device Use While Operating a Motor Vehicle

On Sunday November 21st at approx. 1720 Hrs. I responded to 191 Jackson Rd for a motor vehicle accident roll over with entrapment. Upon arrival I witnessed MV1 on its side in the middle of Jackson Rd. The operator of MV1 was conscious and alert and was taken out of the MV by the Newton Fire Department. The operator of MV1 stated that she was driving S/B on Jackson Rd. and she did not see the parked car on the right side of the road. MV1 then tried to swerve around MV2 and clipped the left rear bumper, and left rear door of MV2 which caused MV1 to roll over onto the middle of the road. MV1 had major damage all around the vehicle and all airbags deployed. MV2 had major damage to the rear and to the left side of the MV. Due to the extensive damage and length of travel of MV1 it was estimated that MV1 was traveling at a speed greater than 35 MPH. Speed limit for Jackson Rd is 25 MPH. MV2 was pushed approx. 20 feet from where it was parked, and MV1 travelled approx 25 feet after on its drivers side after impact. The operator of MV1 was transported by EMS to NWH, and Todys's took possession of both motor vehicles. Pictures were taken and submitted to IT. The operator of MV1 was cited 90/17, speeding.

On November 23rd, 2021 at approximately 14:08 hours while working N491 I responded to #25 Pearl St for a report of a MV crash with airbag deployment. On my arrival I located the involved vehicle a 2021 black Cadillac, Ma reg. K9195 with extensive damage on the property of a four unit condominium complex that abuts #275 Centre St(Plaza). The operator who was standing outside her vehicle was identified as Margaret Cloherty. She appeared uninjured from this crash. She reported the following: She first entered the parking lot to #275 Centre St which has several different businesses. As she drove through the parking lot her vehicle suddenly began to accelerate. She attempted to brake but her vehicle continued straight through the parking lot continuing to accelerate before finally crashing through a wooden fence and onto the property to #21 to #27 Pearl ST. Ms Cloherty was evaluated by the Medics but refused to by transported to the hospital. She signed a patient refusal form. Her vehicle which appeared to be totaled was towed from the scene by Todys. The property owner of the fence was identified as Steve Knapp 508-958-1906 was contacted by police. He will be provided a copy of this report of his insurance.

On Thursday, December 16, 2021, while assigned to Traffic unit N525, I responded to the area of Crafts Street and Linwood Avenue, Newton for a report of a single car motor vehicle crash with entrapment. The weather at the time of the crash was cloudy. The road surface at the time of the crash was wet. Crafts Street and Linwood Avenue are both public ways in the City of Newton. On my arrival, I observed a 2007 Toyota Rav 4 (MA: 7565LR), facing North in the Northbound lane of Crafts Street past the Linwood Street intersection. The vehicle had extensive damage to it's passenger side and was resting on it's drivers side. The lone occupant, Mr. Frank Distefano (S00385929), was trapped in the drive seat of the vehicle and was unable to exit due to his legs being pinned under the steering wheel/dash area. First Responders were able to rescue a pug dog from the vehicle and care for it until a family member responded to take custody of it. The dog did not appear to be injured. Newton Fire responded and had to use stabilizing equipment to support the vehicle during their rescue efforts to make sure the vehicle did not roll over onto it's roof. Newton Fire used specialized extrication tools to remove the roof of the vehicle to gain access to Mr. Distefano. Mr. Distefano was removed from the vehicle by Newton Fire and Medics and transported to Brigham and Women's Hospital in Boston for his injuries. Tody's towing removed the vehicle from the roadway. While Newton Fire was attempting to removed Mr. Distefano from the vehicle, I spoke with a witness to the crash. Mr. John Devalpine (S65213095). Mr. Devalpine stated he was operating his vehicle behind Mr. Distefano on Crafts Street (N). Mr. Devalpine stated he observed Mr. Distefano's vehicle stopped at the light on Crafts Street (N) at Linwood Avenue. Mr. Devalpine stated he then observed Mr. Distefano's vehicle accelerate from it's stopped position and drive across the intersection towards the right side of the roadway. Mr. Devalpine stated the vehicle then crashed into a City of Newton electrical box on the sidewalk. The collision caused the vehicle to turn over and land on it's driver side. I responded to Brigham and Woman's Hospital and spoke with Mr. Distefano in the Emergency Department. His wife, Clara, was present at this time. Mr. Distefano stated he was operating his vehicle and the next thing he knew his airbags deployed and he was trapped inside his vehicle. Mr. Distefano could not recall any other information about the crash. As a result of this crash, Mr. Distefano was cited with Massachusetts Uniform Citation 230024AB for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Due to Mr. Distefano not being able to recall the events that led to his crash, and his recent driver history, I completed and submitted an Immediate Threat with the Registry of Motor Vehicles for Mr. Distefano. This request was emailed to the Registry (DCUImmediateThreat@massmall.state.ma.us). Photos were taken of the crash and submitted to the IT Bureau.

ON 2-4-22 AT APPROX. 1325HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT UPON ARRIVAL AT THE INTERSECTION OF WALTHAM AND PLEASANT I SPOKE TO THE DRIVER OF VEHICLE #1. DRIVER STATES HE WAS HEADING E-BOUND ON PLEASANT ST STOPPED AT THE STOP SIGN. HE STATES VEHICLE #3 SWERVED INTO HIM HITTING THE FRONT OF HIS VEHICLE. THE HIT PUSHED VEHICLE #3 ONTO THE OTHER SIDE OF WALTHAM ST WHERE HE SMASHED INTO VEHICLE #2. VEHICLE #2 STATES HE WAS TRAVELING N-BOUND ON WALTHAM WHEN VEHICLE #3 CROSSED THE YELLOW LINE INTO HIS LANE AND HE WAS UNABLE TO AVOID HITTING HIM. VEHICLE #3 STATES HE WAS TRAVELING S-BOUND ON WALTHAM ST. AS HE APPROACHED PLEASANT ST VEHICLE #1 ENTERED THE ROADWAY HITTING HIS VEHICLE AND FORCING HIM TO THE OPPOSITE LANE WHERE HE SMASHED HEAD ON INTO VEHICLE #2. VEHICLE #1 HAD MINOR LEFT FRONT QTR PANEL SCRAPES AND PAINT TRANSFER. VEHICLE #2 HAD EXTENSIVE FRONT END DAMAGE AND AIRBAG DEPLOYMENT. VEHICLE #3 HAD EXTENSIVE FRONT END DAMAGE AND AIRBAG DEPLOYMENT. DRIVER OF VEHICLE #1 AND #3 REPORTED NO INJURIES AND SIGNED PATIENT REFUSALS. DRIVER OF VEHICLE #2 WAS TRANSPORTED BY MEDIC3 TO NEWTON WELLESLEY HOSPITAL WITH MINOR INJURIES. VEHICLE #1 WAS OPERATIONAL AND LEFT THE SCENE. VEHICLES #2 AND #3 WERE TOWED BY TODYS. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES. CLEARED WITHOUT FURTHER INCIDENT.

On Sunday 2/13/22 at approximately 1746 hours while assigned to marked unit n501 patrolling division n495, I was dispatched to the intersection of Commonwealth Ave and Homer St. for a report of a single car MVA into a traffic signal. Upon arrival I observed Ma Reg 9LL519 on the island facing eastbound in the intersection of Commonwealth Ave and Homer St. The vehicle hit and knocked the traffic signal off its base and landed in the eastbound lane of Homer Street. Operator, O'CONNOR James, states he was travelling Eastbound on Commonwealth Ave and attempted to turn right onto Homer Street. He was unable to pull out of the turn due to the snowy and icy conditions and slid into and knocked over the traffic signal. I was able to move the signal out of the lane of traffic and onto the center island and Dagle Electric was notified. Two pictures were taken and attached to this report. O'CONNOR's vehicle was inoperable and was towed by Tody's. At the time of the accident it was difficult to determine what the damage was. Most serious damage appears to be to the under carriage, rendering the vehicle inoperable.
