On March 10, 2019 at approximately 16:15 hours I responded to Commonwealth Ave and Centre St. for a motor vehicle crash. Operator of MV2 said that he was driving westbound on Commonwealth Ave through the green light and was hit by MV1. The operator of MV1 said that he was traveling northbound on Centre St. and did not see the red light until last minute because a truck was blocking the signals. Both passengers of MV2 complained of minor soreness and refused medical transport. The operator of MV1 was issued a civil motor vehicle citation (T1140496) for MGL90/7 failure to stop at red light.

Operator of vehicle one ( MA reg 3986BV 2012 MERZ ) , Vladimir Krapchev stated that on 03/21/2018 at 08:56 hours he was stopped at a red light on Center St at Commonwealth Ave. Krapchev stated that his vehicle was heading south on Center St and was the third vehicle in line to turn west bound on Commonwealth Ave. Center St and Commonwealth Ave are both public ways in the city of Newton. Krapchev stated that " a red/maroon full size sedan fire department car " struck the front driver's side fender of his vehicle. Krapchev stated that the fire department car was travailing south bound on Center St in the north bound lane, passing stopped south bound Center St traffic. Krapchev stated that there was also north bound traffic on Center St that was turning from the west bound lane of Commonwealth Ave. Krapchev stated that the fire department car turned onto the east bound lane of Commonwealth Ave and did not stop. Operator of vehicle two Newton Fire Department LT Phillip McCully stated that he was driving Newton Fire Department Car 14 ( MA reg MVN MF461 2004 Ford Crown Victoria ) south bound on Center St with his vehicle's emergency lights and siren activated. Lt McCully stated that he was responding to a motor vehicle crash with possible entrapment on Hammond Pond Parkway. Lt McCully stated that he was passing stopped Center St south bound traffic in the north bound lane of Center St. Lt McCully also stated that there was north bound traffic on Center St and " it was tight between the cars ". Lt McCully stated that he was unaware that a crash had occurred. Lt. McCully stated that he did not fell and contact between his vehicle and Krapchev's vehicle. Lt McCully also stated that he did not hear the crash because of his vehicle's siren was activated. Krapchev's vehicle had minor damage and red paint marks on it's front driver's side fender. The Fire Department car had scuff marks on it's passenger side rear door. Neither vehicle was towed and both operators stated that they were not injured in the crash. I took photos of both vehicles and forwarded the disk to the NPD It bureau for down loading.

Mv#1 operator was travelling through the intersection of Commonwealth Ave westerly at Centre St on a green light. The intersection was under construction. Ofc Coletti was working a detail in the middle of the intersection directing four way traffic, waving Comm Ave westerly traffic safely through. Mv#2 operator was stopped in the intersection on Centre St southerly waiting to proceed through. At that time, #2 nosed out into the intersection without being directed and #1 passenger side front sideswiped #2 driver's side front. No injuries. Both MVs appeared to sustain minor damage. #2 operator was unlicensed. #2 towed by Tody's.

MV#1 stated while traveling Southbound on Centre St in the left turn lane at the intersection of Centre and Commonwealth ave. He stated he was at a complete stop waiting to turn left on to Commonwealth ave. He stated MV#2 was in the right lane adjasent to his vehicle when it took a sudden left turn on to Commonwealth Ave striking MV#1 in the right rear side oh his vehicle with the front left end of MV#2. He stated MV#2 then continued to drive behind him Eastbound on Commonwealth ave. MV#1 stopped on Read CT to evaluate the damage at which point MV#2 kept driving. MV#1 stated the car traveling behind MV#2 witnessed the incident and gatherd the plate information. The witness was not on scene upon my arrival nor did MV#1 gather their information. Newton Dispatch ran MA Reg 7EW346. The plate came back to a Black Honda Civic 2005 matching MV#1's description of the vehicle. The owner of the Civic was listed out of Brighton. I followed up at 78R Foster St Brighton, Upon arrival I noticed a black Civic matching the plate and description. The Civic had damage to the front end and damage to the back right side. I spoke with owner of the vehicle that stated he had not been driving today and had been at home. He did not know who was operating the vehicle at the time. Speaking with Medina the owner of MV#2 there was a significant language barrier issue. I will not be charging owner of MV#2 due to the vehicle damage not being consistent with MV#1 statements, as well as proof of operation. MV#1 was not towed. Operator of MV#1 stated he did not sustain any injuries at this time. Owner of MV#2 did not have any injuries to him consistent with being in a MV accident.

On Wednesday 7/17/2019 at approx 1635hrs, while assigned to N-494, I responded to the intersection of Comm Ave @ Centre St for a 2 car MVA. Operator of MV1 states she was traveling EB on Comm Ave. She states that while attempting to travel straight across the intersection MV2 cut right in front her striking the front passenger side of MV1. Operator of MV2 states that she was traveling WB on Comm Ave as she approached the intersection. She states that she didn't see incoming cars as she attempting to take a left onto Centre St. She states that MV1 "came out of nowhere" and must have been speeding." MV1 (MA Reg: 8KF367; 2016 Lexus RX350) was traveling SOUTH on Centre St. Operator of MV1, Rostyslav Levener, stated that he began to slow in traffic as he approached a red light at Commonwealth Ave. While slowing down to about 5mph, MV1 was struck by MV2 from behind. MV2 (MA Reg: 45z480; 2008 Chevy Impala) was traveling SOUTH on Centre St when it collided with MV1. Operator of MV2, Erez Meyer Hatch-Tuchman, did not gauge the distance between his vehicle and MV1 in front of him. MV1 was damaged on the driver's side bumper. MV2 had damage to its front passenger bumper, passenger door and side mirror. No injuries on scene. Operator Erez's parents arrived on scene and took possession of the vehicle.

On 11/29/2019, while assigned to N494, I, Officer Conary, and Detective Harris (N503), responded to Commonwealth Ave at Centre Street for a MVA. Upon arrival, I met with operator of MV1 who stated that he was traveling Northbound on Centre Street, when he went through the red light and hit MV2. Operator of MV1 said that he wasn't paying attention when he went through the red light. Operator of MV2 stated that he was traveling Westbound on Commonwealth Ave when MV1 hit MV2. Operator of MV2 explained to me that he had a green signal on Commonwealth Ave and that MV1 went through the red light at Centre Street. Both cars were towed. All parties were evaluated by medics and signed patient refusals. Operator of MV1 was issued a warning MA Citation #T2079475 Ch89/9 Failure to Comply with Red Signal. No further incident to report.

Mv#1 was traveling north on Centre Street when it was involved in an argument with Mv#2, resulting in the motor vehicles colliding(ref inc 19055516). -OpMv#1 stated that after the confrontation Mv#2 cut him off striking his vehicle. -OpMv#2 stated that after the confrontation Mv#1 was trying cut him off striking his vehicle. - Mv#2 Pass1 and Mv#2 Pass2 statements coincide with those made by OpMv#2. -Wit#1 he observed Mv#1 and Mv#2the at the intersection of Commonwealth Street and Centre Street. Mv#1 on the right lane and Mv#2 on the left turning lane. He stated the parties exchanging words at the intersection and a few minutes both parties spat on each other's vehicles. When the traffic control light changed he observed Mv#2 speed off and Mv#1 suddenly changed lanes after it. They stated that they observed that when both vehicle were on Commonwealth Avenue Mv#2 cut off Mv#1, causing the vehicles collided. Mv#1 threw a cup of coffee at Mv#2. Wit#1 and Wit#2 stated they intervened and de-escalated the situation until police arrived. - None of the parties reported injuries to them at the scene not either vehicle was towed. Op MV#1 was cited for Reckless Operation Ch 90s24. (Cit#2079508) by suddenly changing lanes and cutting across the intersection to follow Mv#2. Op MV#2 (Cit#2079507) was cited because his driving caused the collision by shifting lanes with out signaling, cutting off MV#1.

On 03/11/2020, while assigned to N495, I, Officer Conary, responded to the intersection of Centre Street at Commonwealth Ave for a minor motor vehicle accident. Upon arrival, MV1 passenger was getting evaluated by the medics. I spoke to the Operator of MV1 and he stated he was traveling Northbound on Centre street and stopped at the red light at the intersection. He began to roll forward only a couple feet then he came to a complete stop again. At this time, there was a green left signal only for the left lane only. MV2 hit MV1 from the rear. Operator of MV1 said the other driver was distracted. Operator of MV2 traveling Northbound on Centre Street, behind MV1, came to a stop at intersection. She saw the green left signal only and proceeded forward hitting MV1. Operator of MV2 said that she wasn't paying complete attention. Both vehicles were able to be driven from scene. There was minor damage to MV1 rear bumper. There was minor damage to MV2 front bumper. MV1 passenger signed a patient refusal. No further incident to report.

Operator of MV1 (Ma Reg 3NWW40) stated she was traveling NB on Centre Street when she came to a stop light at the intersection of Commonwealth Ave. While waiting for the light her vehicle was struck in the rear by a school bus. Operator of MV2 (Ma SBN 42204) stated he was traveling NB on Centre Street approaching the intersection of Commonwealth Ave. Operator of MV2 stated that he stopped behind MV1 but was too close and he bumped the rear of the vehicle. The damage was minor to MV1 and MV2 had no visible damage. No parties claimed any injuries at this time. MV2 is a yellow school bus that was empty at the time but was being used as a shuttle service for Boston College. Both vehicles were able to drive off from the scene safely.

MV1 stated they were facing southbound on Centre St. sitting at a red light at the intersection of Commonwealth Ave. when they felt their vehicle was hit lightly from the rear by MV2. MV1 sustained no injuries or damage to their vehicle. MV2 stated they were stopped at a red light facing southbound on Centre St. at the intersection of Commonwealth Ave. MV3 hit the rear of MV2 and the vehicle went forward and lightly tapped MV1. MV2 had no injuries and minor damage to the rear end of the vehicle. MV3 stated they were stopped at a red light facing southbound on Centre St. at the intersection of Commonwealth Ave. when MV4 crashed into the rear of their vehicle. MV3 sustained heavy rear end damage, minor front end damage and was towed by Tody's. In the vehicle was a sick cat, which was transported to the Vet by Newton Police Animal Control. Passenger in MV3 was taken to Newton-Wellesley Hospital for minor injuries. A towed MV form was completed, MV3 took all valuables prior to tow. MV4 stated they were traveling southbound on Centre St. towards the set of lights at Commonwealth Ave. MV4 stated they attempted to hit the breaks for the red light and rear ended MV3. MV4 sustained minor front end damage. MV4 was issued MA Citation #T2014419 MGL Ch.90 s11 License not in Possession. Photos were taken of the scene and sent to IT.

MV1(Corona) was traveling southbound on Centre St through the intersection of Centre St at Commonwealth Ave. MV2 (VAZ) was traveling eastbound on Commonwealth Ave through the intersection of Centre St at Commonwealth Ave. Both vehicles collided at the intersection. The operator of MV1 stated she did not know what happened at the time of the crash and might have gone through the red light, the operator of MV2 stated the same. The operator of MV2 stated he spoke with the operator of MV1 and she stated to him that she was looking at her GPS. There was heavy front end damage to MV2 and passenger front and side damage to MV1. A witness (Danielle Hite), was two car lengths behind MV2 stated that their line of traffic had a green light and that she saw MV1 drive through a red light causing the collision. Fallon Medics transported the passenger of MV2 to Beth Israel as she was lying in the road unconscious and the medics obtained a patient refusal from the operator of MV1 and the operator of MV2. Both vehicles were towed by Tody's.

MV1 operator states she was stopped at the traffic light on Commonwealth Avenue when MV2 was on Centre Street turning right eastbound slid from the ice roadway into the front end of MV1. MV1 operator had no injuries and the vehicle sustained mild damage to the front of the vehicle. I spoke to Operator 2 on the phone. MV2 operator states he was turning right onto Commonwealth Avenue when his vehicle lost control because of the slick roadway. MV2 operator stated he was at fault. MV2 had very little damage to the front of the vehicle due to it being a larger vehicle. MV2 operator had no injuries. No tow was needed for either vehicles. Operator 2 shared information with operater 1, he left prior to our arrival.

MV1 Operator states she was traveling southbound on Centre Street when an orange Chevrolet Camaro rear ended her vehicle. MV2 did not stop for the accident and continued driving. The road ways were slick from ice. MV1 operator had no injures. I attempted to contact MV2 operator but was unsuccessful. Due to there being minimal to no damage to MV1 we will not be seeking charges for MV2 Operator for leaving the scene of property damage at this time. Traffic has been updated to follow up with this report. Traffic Bureau update (Officer Gaudet): I attempted to make contact with the registered owner of MV2, Mr. Jorge Ramirez S17331100, with a negative result. A "Hit and Run" inquiry was mailed to the address listed for Mr. Ramirez with the registry.

MV1 Operator states he was at the intersection of Commonwealth Avenue and Centre Street traveling eastbound. MV1 Operator stated the green light for the left turn only was activated. The green light to go straight ahead for the middle lane was not activated. MV1 Operator states he was then rear ended by MV2. MV1 Operator stated the other vehicle drove away without stopping to exchange information. MV1 sustained damage to rear center. MV1 Operator had no injuries. Dispatch stated MV2 (MA REG #W63589) had no MA OLN on file for the vehicle. I was unable to find or contact anyone who owned MV2. No injuries, no tows. Traffic Bureau update (Officer Gaudet): MV2 is a black 2011 Mercedes E350 (MA CON: W63589) . This vehicle is registered to a LAJANSLIM Services Inc out of 346 Eastern Avenue in Lynn, Ma. I attempted to located a contact number for this company with a negative result. On February 15, 2022, I contacted the listed insurance carrier for MV2, Commerce Insurance. Commerce could not provide any contact information for the registered owner of the vehicle, but they were able to provide contact information for the vehicles insurance agent, Friendly Insurance. I contacted Friendly Insurance with an address out of Lynn, Ma. An agent there was able to provide contact information for the owner of MV2, Mr. Fritherson Jean-Baptiste (S50659798). I contacted Mr. Jean-Baptiste who confirmed he is the owner of the 2011 Mercedes E350 involved in the crash. Mr. Jean-Baptiste stated he was not operating his vehicle at the time of the crash and stated it was someone that works for him. Mr. Jean-Baptiste was not able to provide the name of the operator of his vehicle at the time of the crash, but stated he would look into it and contact me with that information. Mr. Jean-Baptiste contacted me later and the day and said he was half asleep when we first spoke. I explained to Mr. Jean-Baptiste again why I contacted him regarding his vehicle. Mr. Jean-Baptiste stated his girlfriend has COVID and he has been quarenteening with her in Quincy. Mr. Jean-Baptiste stated his car was parked in Cambridge and to his knowledge no one else has operated it. Mr. Jean-Baptiste stated he did not understand what was going on with his vehicle and stated he now does not know where it is. Mr. Jean-Baptiste stated he believes his vehicle may now be stolen. Mr. Jean-Baptiste was advised that if he believes his vehicle was stolen and he does not know who was operating it at the time of the crash on February 12, he needs to contact the Cambridge Police Department and report the vehicle stolen. I advised Mr. Jean-Baptiste that I would follow up with him about his vehicle and it's operator. Officer Perry spoke with a witness to the crash, Mr. Robert Drabiuk. Mr. Drabiuk stated he was at the intersection of Commonwealth Avenue and Centre Street on foot when he observed the crash. Mr. Drabiuk stated to Officer Perry he observed MV2 rear end MV1. Mr. Drabiuk also stated the operator of MV2 got out of the vehicle after the crash, put his hands in the air, and then drove away without exchange of information. Mr. Drabiuk stated he observed a black Mercedes (MA REG # W63589) drive off and stated there was a black male operating the motor vehicle. Traffic Bureau update (Officer Gaudet): As of February 28, 2022, I have attempted to contact Mr. Jean-Baptiste numerous times to follow up on his vehicle's involvement in this crash. A party answers the phone and after I identify myself they hang up. There is no record to date of Mr. Jean-Baptiste reporting his vehicle stolen. At this time, I am unable to indeifty Mr. Jean-Baptiste as the operator of the vehicle involved in the crash.

Operator of MV1 stated she was traveling straight ahead, eastbound, on Commonwealth Avenue when a red sedan struck her vehicle. Operator states she was driving through a green light signal when Operator of MV2 went through a red light signal. Operator of MV1 said she pulled over to exchange information with MV2 Operator when she observed the party get out of the vehicle gather a piece of the broken MV2 vehicle and then drive away. MV1 sustained damage to the front right of the vehicle. Operator of MV1 signed a refusal from the medics on scene. No injuries, no tow.

On 02/27/2022, while assigned to N494, I, Officer Conary, and Officer Fay (N495), responded to the area of Commonwealth Ave at Centre Street for a two car MVA with airbag deployment. Upon arrival, I observed MV1 head on into MV2 front end in the middle of the intersection. I spoke with Operator of MV1 who stated that she was traveling Eastbound on Commonwealth Ave about to take a left onto Centre Street Northbound. She was in the left lane, and had a left turn green arrow. She proceeded when she hit MV2. Operator of MV2 stated that they were traveling Westbound on Commonwealth Ave when they were hit by MV1. They said that they had a green light to proceed straight through the intersection. While on scene, I observed the lights cycle through properly. Both vehicles were towed from scene. Both parties declined medical attention. Operator of MV1 was under 18, and had her parents respond on scene.

On 5/11/22 I responded to Commonwealth Ave. and Center St. for a 2 car accident. Upon arrival I spoke with the operator of MV1 who stated he was at fault for the accident. He stated that he was on the center lane of Comm. Ave. heading w/b when he noticed that he had to take a right turn. He stated that he checked his mirror and reversed to merge onto the right lane, and that is when he hit MV2 which was behind him. The operator of MV2 stated that he was driving e/b on the center lane when MV1 backed up and hit him causing moderate damage to the right front of the vehicle. Both parties stated they had no injuries and denied medical attention. Both parties were given the report number and gieven information on how to get a copy of the report.

On July 4, 2022 at approximately 06:56 hours I, Officer Guarino, responded to the area of Commonwealth Ave at Centre St for a report of a motor vehicle crash. Upon arrival, I observed the fire and medics on scene with a two vehicle motor vehicle crash, one of which was a rollover. MV1 was on the roof of its car in the northbound lane of Centre St. MV2 was in the Eastbound Lane of Commonwealth Ave. I spoke with the operator of MV2, Annika Morris-Kelly who told me that she was traveling Eastbound on Commonwealth Ave when she stopped at the red light. The light turned green and she began to go, as she proceeded, she noticed MV1 traveling southbound on Centre St and tried to stop but crashed into MV1. I spoke with the operator of MV1, Luymar Bezerra, who said that he was traveling southbound on Centre St and did not see the traffic signals due to the trees on the side of the road and continued through the intersection causing the crash. I asked Luymar for his license and registration. He provided me with a passport and a CT registration for his vehicle. Luymar said that he did not have a license on his posession and did not have a valid license within the United States. He said that he had a Portugese driver's license and has been in the country for less than a year. I was unable to confirm this information and issued a criminal complain against Luymar Bezerra on MA Uniform Citation T1447651 for MGL 90/10 Unlicensed Operation of a motor vehicle. I explained the citation to Bezerra, who said that he understood. All partied involved were seen by medics and signed a patient refusal. Both vehicles were towed from the scene by Todys.
