Narrative:
At approximately 0010HRs on Thursday, June 13, 2019, I was dispatched to 141 Dedham St. for a report of a two car crash. Upon arrival, I observed MV1 (NH Pass:3266403) to be parked on the side of the roadway in front of the aforementioned address and it appeared to have been rear ended by MV2 (MA Pass: 513WA3). Being that MV2's airbags were deployed, I requested Newton Fire to evaluate the operator of MV2, who was ultimately medically cleared and signed a patient refusal. Upon speaking to the operator of MV2, he stated that he was traveling Northbound on Dedham St. and feared that he was coming too close to the solid yellow divider lines and over steered to the right, accidently hitting MV1, which was parked. The owner of MV1 was on scene and stated that he was inside and came out to find the crash. I noted damage to the driver's side quarter panel of MV1 and damage to the front end/hood area of MV2. Todys serviced arrived and took custody of MV2. I completed/filled an NPD Towed Motor Vehicle Form. I cleared without incident.

Vehicle #1 was traveling South on Waverly Ave. Vehicle # 2 was attempting to enter the Southbound lane of travel on Waverly Ave from Franklin St. Vehicle #2 pulled out into the lane that Vehicle #1 was already established in. Vehicle #1 had no time to stop and struck Vehicle #2 on the drivers side front wheel well. Both vehicles sustained damage. Vehicle #1 had major front end damage and Vehicle #2's tire was bent in. Vehicle #2 was towed from scene by Todys. No injuries reported on scene. Vehicle #2 operator was cited citation # T034419 89-9 failure to use care when entering intersection.

Operator of MV1 states she arrived at The Sinclair restaurant in Cambridge with a friend at approximately 11:30pm and had 1 glass of wine. Operator of MV1 states she left at closing at approximately 2am to drive her friend home located on Mass Ave. Operator of MV1 states after dropping her friend off she traveled home to 41 Hyde St in Newton. Operator of MV1 states she took Lake Ave westbound heading towards Hyde St when she was fighting off sleep and must have passed out at the wheel and struck a city tree on the eastbound side of Lake Ave in front of 160 Lake Ave. Operator of MV1 states she contacted AAA at 3:37am, but they never showed up so she abandoned the vehicle and walked home. Operator of MV1 states she didn't sustain any injuries and refused medical attention. MV1 sustained heavy damage to the left front fender and was towed by Tody's.

On 06/30/2019 at 15:30 hours I was notified by Sgt Lee that a crash involving a motor vehicle and a bicyclist had occurred on Wiswall Rd at M Roadway. Upon my arrival at the crash scene I made the following observations, Wiswall Rd is a two lane non divided public way in the city of Newton that runs north to south from a dead end to Dedham St. The posted speed limit on Wiswall Rd is 25 MPH. M Roadway is a two lane non divided public way that runs west to east from Wiswall Rd to a dead end cul-de-sac. The weather conditions at the at of the crash were cloudy, and the roadway surface was wet from rain. I observed MA commercial registration M82794 a gray 2014 Honda Accord stopped in the north bound lane of Wiswall Rd at M Roadway. The Honda is registered to Newton Driving School. I observed two smudge marks approximately four inches in length on the center portion of the front and rear passenger side doors of the Honda. I also observed a 20 inch Kent bike that had be moved from it's final point of rest prior to my arrival. The bike had damage to it's front handle bars, front tire and it's two front forks. The bike's front tire had gray scuff marks on it. At the crash scene I spoke with the operator of the Honda, David Dombrovsky. Dombrovsky stated that he was driving north bound on Wiswall Rd near M Roadway approximately 20 to 25 MPH. Dombrovsky stated that he has had his learner's permit for six months and is a student at the Newton Driving School. Dombrovsky stated that he did not see the bicyclist but heard the impact of the bicyclist stricking the passenger side of the Honda. At the crash scene I spoke with driving school instructor, Igor Drizik. Drizik stated that the Honda was driving north on Wiswall Rd near M Roadway at approximately 20 to 25 MPH . Drizik stated that he did not see the bicyclist until approximately 1 to 2 seconds before impact on the driver's side of the Honda. Drizik stated the Honda is equipped with dual brakes and that he applied the brakes at the same time as Dombrovsky. Drizik stated that he and Dombrovsky got out of the Honda to render aid to the bicyclist. There were three other driving school students in the rear of the Honda at the time of the crash, neither of them stated that they witnessed the crash. None of the occupants of the Honda stated that they were injured and the vehicle did not require a tow. The bicyclist was identified as David Kerwin-Derrick. Kerwin-Derrick was transported via EMS to Boston Children's Hospital form treatment. At the hospital I spoke with one of Kerwin-Derrick's mother's, Patricia Kerwin-Derrick, who stated " We all know that David rode his bike into the car " and that her son had a broken left leg. Kerwin-Derrick is a student at the Memorial Spaulding School. Based upon my observations of the crash scene and statements made to me , I believe that Kerwin-Derrick's unintentionally rode his bike into the passenger side of the Honda. Photos were taken of the Honda, bike and crash scene and the disk was down loaded by the NPD's IT Bureau.

On Monday, July 1st 2019 at 0740 hours, I responded to a single car Motor Vehicle Accident outside the area of 140 Vine St. The vehicle was a 2016 Black Merz Mass Reg # 5AX136 being operated by 17 year old Eleanor Grace Bomberg. The vehicle was headed north on Vine St when it veered off the side of the road and collided with a city tree outside of 140 Vine St. The vehicle flipped over and landed in the middle of the street. The vehicle suffered extensive damage to the front end and right side. The operator suffered minor injuries to her arm. Eleanor's father, Patrick Bomberg, signed the patient refusal. Todys's towed the vehicle. Pictures were taken and submitted to the IT Bureau.

On 07/02/2019, while assigned to N494, I, Officer Conary, and Officer Anderson (N502) responded to Kenrick Street for a MVA. Upon arrival, I met with witness GRAU, who told me that he observed MV1 traveling Westbound on Kenrick Street. MV2 was exiting Dorr Road onto Kenrick Street and quickly went across the lanes of traffic and hit the stone wall at 337 Kenrick Street. MV1 swerved and hit MV2 on the front right side. Operator of MV1 stated that he was traveling Westbound when MV2 quickly exited Dorr Road and went across Kenrick Street. Operator of MV1 swerved but hit MV2 on the front right side. MV1 had heavy front end damage. Operator of MV2 stated that she stopped on Dorr Road waiting to turn onto Kenrick Street. MV2 began to roll back slightly due to being on a hill. Operator of MV2 pressed the gas to move up but pressed it too much which caused her to accelerated across Kenrick Street. MV2 then hit the stone wall at 337 and then MV2 collided with MV1. MV2 had heavy front end and right side damage. All operators and passengers were offered and signed patient refusals from the medics. Both motor vehicles were towed from the scene. Pictures were taken of the property damage to be submitted to IT accordingly. No further incident to report.

MV1 was traveling SOUTH on Chestnut St when he proceeded through a green light showing at the intersection. When he entered the intersection at Commonwealth Ave he was struck by MV2. MV2 had a stop sign on Commonwealth Ave in the carriage lane and proceeded into the intersection resulting in a collision with MV1. MV1, a 2010 Toyt Scion (RI Reg: ZI954), was being operated by a Kebin Eduardo Morales Pena. MV2, 2013 Subaru Legacy (MA Reg: 616WF8), was being operated by a Leonid Natanzon. There was heavy front end damage to MV2 and the driver's side back door was crushed in on MV1. Airbag deployment on MV1, Pena signed a refusal with the medics and his vehicle was towed by Tody's. Natanzon was uninjured and was issued in hand MA Uniform Citation T1441751 for 89/9 - Stop Sign, Fail.

OPMV#1 Was traveling Eastbound on Washington St when they were struck by the OPMV#2 exiting the NWH at 2014 Washington St. OPMV#1 Stated he was traveling straight on Washington St heading home and noticed a vehicle pulling out into his lane then stop. The operator of MV1 (ma reg RWH529) was traveling Southbound on Centre St when he observed a disabled truck in the middle of the street. The truck was stopped in the middle of the road at the intersection of Cabot St. Operator of MV1 stated he attempted to go around the truck entering the NB lane but had to quickly cut back into his lane because of oncoming traffic. Operator of MV1 caught the corner of the metal foot bar of the traffic ripping his whole left passenger side open. Operator of MV2 (CT apportioned 49135A) stated while driving his vehicle SB on Centre St on his way to Needham he could feel the truck start to have issues. Operator of MV2 stated while stopped at the light at Centre and Cabot he heard a piece from his truck break off. The break became disabled at this time and could not be moved. Operator of MV2 called the police to report the issue, before we arrived Operator of MV1 had caught the edge of the metal foot hold. Both vehicles were towed by Tody's. Neither driver reported any injuries at this time. There was no visual damage to MV2. MV1 sustained heavy body damage all along the passenger side.

On Tuesday July 9th 2019 at approximately 1409 hours I responded to a two car accident on Kenrick St and Waverley Avenue. At the time the weather was sunny and the road conditions were dry. Kenrick St and Waverley Ave are both public ways owned by the City of Newton. The operator of MV1 identified as Smreeti Khanal (S82370804) stated she was stopped at the red light on Kenrick Street and was waiting to turn left onto Waverley Ave when her vehicle MA REG 623KD7 was struck by a large box truck that was turning left onto Kenrick Street from Waverley Ave. MV1 sustained heavy front end damage and was towed from the scene by Todys. Two small children were in the back seats during the accident. No injuries were reported. The operator of MV2 identified as Keiven Vandon Stewart (TX 33693636) stated he had the green light and was turning left onto Kenrick St from Waverley Ave and the side of his truck made contact with MV1. MV2 stated MV1 was not on the right side of the road and did not leave enough space for the truck to make the turn. MV2 reported no injuries and sustained no damage. AMB1 responded and cleared with four patient refusals. MV2 was able to leave the scene.

On Tuesday 7/9/19 at approximately 1734 hours while assigned to marked unit n499 I was dispatched to the area of 76 Nahanton Street for a report of a 2 car MVA with no injuries. Upon arrival I spoke with the operator of MV1, BISHOP Robert, who stated that he was travelling eastbound operating a loner vehicle from Mercedes of westwood when the vehicle began "bucking" back and forth as he was slowing to a stop. He stated he didnt know why the vehicle was operating in such an erratic manner. Robert further stated that he was concerned that he would strike the vehicle in front of him so he swerved toward the middle of the road crossing the double yellow line into the path of MV2 striking the vehicle. I also spoke to the operator of MV2, LEMBERG Ekaterina, who stated that she was travelling westbound when she saw MV1 cross the double yellow line into her lane of travel. She was unable to stop and was struck by MV1.

Upon arrival I spoke with the operator of RI Reg FV705, Malorie CURLEY who stated she had parked her new work vehicle in the parking stall was exiting her vehicle to see if she was within the lines when the operator of Ma Reg 69EL83 Laurence HENDY pulled into the spot next to her, opened his door striking her truck, CURLEY stated as she was rounding the car HENDY proceeded to grab his door and slam it into her car several more times. CURLEY stated she confronted HENDY who refused to exchange information with her for the damage to her car and fled. CURLEY took pictures which she will provide in her own crash report. The front bumper of CURLEYS car had several spots where the white paint was peeled back , HENDYS vehicle had the matching white paint on the edge of his drivers side door. Although the damage was minor CURLEY requested a report for documentation as it was her work vehicle. i attempted to contact HENDY but no phone number is listed.

On 07/12/19 at 11:36 hours, I responded to Elliot and Oak Streets for a two car motor vehicle accident with no reported injuries. That intersection has a three way stop sign for all travel lanes. Operator of MV1 states he was traveling eastbound on Elliot Street and stopped for the stop sign at Elliot and Oak Streets. After, he proceeded straight through the intersection and was struck by MV2 on his passenger side rear wheel area. He states that MV2 never stopped for their stop sign and was coming through the intersection from Oak Street. I observed passenger side rear wheel and bumper damage to this vehicle but it was still driveable. Operator of MV2 states that she could not recall exactly what occurred but that she stopped for the stop sign. She could not provide which way she was traveling only stating that she was attempting to turn around and pull over to right side of the road to park. I observed damage to the driver's side front bumper area but the vehicle was still driveable. Based on the debris in the road and damage to the vehicles, it appears that MV2 never stopped for the stop sign on Oak Street (NB) as the other operator was proceeding through. MV2 then angled the vehicle to the right in order to pull over and park on the other side of the road. While doing so, she struck MV1. The operator was issued MA Uniform Citation # T1444918 for a Stop Sign Violation.

Veh #1, MA reg 628BJZ, being operated by Mr. David McDonald was traveling east on Commonwealth Ave, attempting to turn right on to Auburn St. According to Mr. McDonald, he began to turn right when he heard a thump against the driver's side of his car, and as he looked, notice a man, Mr. Kejie Gu, fall away from his car. Mr. McDonald pulled over and assisted Mr. Gu to his feet. Mr. Gu got up and walked away. A witness, Mr. Brian Washburn was on his bicycle, and observed the accident. Mr. Washburn was across the intersection traveling west, when he said all the traffic east and west was stopped. Mr. Washburn observed Mr. Gu crossing the street, and stated as he got approximately 2/3 of the was across, he was spun around by the black car. Mr. Washburn believed Mr. Gu to be in the crosswalk. Another witness, Mr. Walter Sims stated he did not see the collision, but did observe Mr. Gu on the ground in the middle of the intersection, not near the crosswalk. Paramedics arrived on scene, Mr. Gu had some scratches on his right elbow, which were bandanged. Mr. Gu signed a patient refusal.

On Monday, July 15, 2019, while assigned to Traffic unit N525, I responded to the area of Beacon Street and Paulson Road, Newton, for a report of a hit and run motor vehicle accident involving a bicyclist. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street and Paulson Road are both public ways maintained by the City of Newton. I spoke with the injured bicyclist in front of 165 Paulson Road, Alexander Mendonez. Mendonez stated he was traveling Westbound on Beacon Street on his bicycle near Paulson Road. Mendonez stated he observed a late model gray sedan "inch to a stop" on Beacon Street (W) at Paulson Road. Mendonez stated he did not remember if the vehicle had it's right directional on at the time. Mendonez stated as he was traveling through the intersection with the vehicle on his left, the vehicle took a right turn onto Paulson in front of him. Mendonez stated he attempted to brake and veer to the right of the vehicle, but was unable to avoid the collision. Mendonez stated his left side made contact with the rear passenger door/rear fender area of the vehicle. Mendonez fell to the roadway and the vehicle left the area Northbound on Paulson Road. Mendonez was unable to see who was operating the vehicle. Newton Medics responded and treated Mendonez for injuries to his left knee/arm area. Mendonez signed a patient refusal with Newton Medics. I observed minor damage to the left handlebar area of Mendonez's bicycle (black Trek). Mendonez was wearing a white helmet at the time of the accident with a blue shirt and white and black shorts. Pictures were taken of the area and bicycle and submitted to the IT Bureau. Mendonez was able to provide a partial Massachusetts registration of "7BS32". I attempted run this plate with multiple combinations. I searched the area for a vehicle matching the description given by Mendonez with a negative result. On July 16, 2019, I spoke with a witness to the incident Mark O'Hearne. O'Hearne stated he was traveling Westbound on Beacon Street a few car lengths behind where the accident occurred. O'Hearne stated he observed a bicyclist on the right side of the road abruptly stop and fall to the roadway. O'Hearne stated he observed a vehicle in the path of the bicycle. O'Hearne stated the vehicle did not stop after the bicyclist fell and took a right onto Paulson Road from Beacon Street. O'Hearne stated he followed the vehicle involved in the accident to Commonwealth Avenue, and while doing so took a picture of the vehicle's license plate. O'Hearne stated he did not observe who was driving the vehicle. The Massachusetts registration provided to me by O'Hearne is 7BS322. This vehicle comes back to a 2010 Volkswagon Jetta registered to Peter Giorgi (S20952090) out of Paxton, Ma. I spoke with Peter via telephone. Peter acknowledged he is the registered owner of the vehicle, but stated his daughter was using the vehicle and was in the Newton area. I spoke with Lindsey Giorgi (S35711247) via telephone. Giorgi stated she was operating MA: 7BS322 in Newton yesterday, but does not recall being in an accident. Giorgi stated she is not familiar with the area and does not know the area where the accident occurred. Giorgi stated there is no damage to her vehicle and stated she does not believe she hit anyone. Giorgi emailed me 4 photos of her vehicle where I described the damage would be. I observed a small area of minor damage to the rear passenger side door area of this vehicle. After speaking with all parties involved, I am requesting a hearing in Newton District Court for Chapter 90, Section 24 (Leaving the Scene, Personal Injury). Giorgi was advised and mailed Massachusetts Uniform Citation T1269847. All photos obtained from O'Hearne and Giorgi were submitted to the IT Bureau.

On 07/16/19 at approximately 09:38 I responded to a two vehicle crash on Needham St. outside of #141. The Operator of Veh #1 stated she was turning left on Needham St when the South bound traffic stopped to left her turn. As she turned, Veh#2 drove South down the multi-turn lane and they collided nearly head on. Heavy damage and air bag deployment to Veh #1. The operator was checked out by Medics and signed a patient refusal. The Operator of Veh #2 stated she was driving down the multi-turn lane South on Needham St.. She was preparing to take a left onto Jaconnet St when Veh #1 pulled out of parking lot and collided with her vehicle. I observed moderate to heavy damage to Veh #2 and noted that the vehicle didn't have its directional on. The operator of Veh #2 complained to left leg pain and was transported to Newton-Wellesley Hospital By Ambulance #1. Both vehicles were towed by Tody's Towing. At the conclusion of my investigation it appears as if both vehicles were at fault during this crash; Veh#1 for not yielding to right of way and Veh #2 for traveling too soon in a turn lane, no directional and driving too fast for the heavy traffic conditions. Both operators were given verbal warnings and there will be no citations issued at this time.

Operator of MV1 was originally attempting to make a delivery to Stop & Shop on Needham Street but could not because the road was closed due to a water main break. The operator then continued (SB) on Winchester Street and took a left on to Rachel Road attempting to turn around using his gps. He then turned left on to Upland Avenue which ends up being a dead end or you can travel up Druid Hill Road. He then attempted to turn the tractor trailer around because he did not want to continue through the residential neighborhood. Somewhere in the process, his truck and semi trailer got caught up in low hanging utility wires. The wires then got pulled down and it split Verizon utility pole 740 in half. This pole is in front of 337 Upland Avenue and it also pulled down the electrical service attached to that residence. These wires run to Verizon utility pole # 1001/7 in front of 66 Druid Hill Road. No damage observed to this pole. There also appeared to be no damage to the truck/semi trailer.

On 07/18/2019 Gladys Bisono came into NPD Headquarters to report a past-hit and run. On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

Tody's Towing took possession of MV#1 and AAA took possession of MV#2. The operator of MV#2 provided me with a witness's information (Atenia at 617 981-0165). I attempted to contact the witness with negative results and no voicemail.

The operator of MV#1 stated she was travelling southbound on Centre St in the left side travel lane and attempted to switch lanes into the right side travel lane. The operator of MV#1 stated MV#2 was in her blind spot and made MV#2 swerved into MV#3. MV#1 never made contact with MV#2. There were no reported injuries to the operator of MV#1 or its passenger. There were no damages to MV#1. The operator of MV#2 stated he was travelling southbound on Centre St in the right side travel lane when MV#1 abruptly entered his travel lane causing him to swerve into MV#3. MV#2 sustained heavy front right side damages. There were no reported injuries to the operator of MV#2. MV#3 was unoccupied and parked in front of 1211 Centre St. MV#3 sustained moderate damages to its left side. The owner of MV#3 was advised of this accident. Based on the statements made to me, I will mail citation #T1268347 to the operator of MV#1 for violations of c89 4A marked lanes violation.

Narrative:
On Tuesday, July 23, 2019, while assigned to Traffic unit N525, I responded to the intersection of Beacon Street and Washington Street, Newton for a motor vehicle accident involving a bicyclist. The weather at the time of the accident was overcast. The road surface was dry. Beacon Street and Washington Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Paige Mahoney (S85200426). Mahoney stated she was stopped in traffic at a red light on Beacon Street (W) at Washington Street in her 2018 black Volkswagon Tiguan (MA: 633MV6). Mahoney stated while her vehicle was at a complete stop, she felt and heard something hit the rear of her vehicle. Mahoney exited her vehicle and saw a bicyclist on the roadway. I observed minor damage to the rear middle bumper area of MV1. There was a small amount of blood present on the damaged area. I observed the roadway where the accident occurred. There was a small pool of blood in the left lane of Beacon Street (W) where the bicyclist fell to the roadway. The area of the roadway where the accident occurred is currently undergoing restoration and the asphalt has been graded down to it's sub base in preparation to be resurfaced. The current road surface condition is bumpy and uneven. The area where the roadway goes from a smooth surface to a bumpy surface is where the accident occurred. The injured bicyclist was identified as Timothy Johnson (S29270605). I spoke with him in the Newton Wellesley Hospital emergency room. Johnson stated he was operating his bicycle Westbound on Beacon Street on the right side of the left lane. Johnson stated he was coming to a stop in traffic at a red light when his wheel abruptly turned to the left and he began to fall over. Johnson stated his head hit the back of a vehicle stopped in front of him and he landed on his right side, with his bicycle landing on top of him. As a result of the accident, Johnson suffered an injury to the right side of his head and had scrapes on his right arm. Johnson stated he did not lose consciousness as a result of the accident and he appeared to be alert and orientated while I was speaking with him. Johnson was wearing a helmet at the time of the accident. There were fresh markings on the right side of the helmet with minor black paint transfer. These markings are consistent with Johnson's statement that he fell into the back of MV1 and hit his head. Johnson's bicycle was removed from the roadway prior to my arrival and was being stored in the driveway of 2044 Beacon Street by the resident there. I observed no obvious damage to Johnson's bicycle. After speaking with all parties involved and examining the roadway where the accident occurred, it appears that the condition of the road surface contributed to Johnson losing control of his bicycle and crashing into the rear bumper area of MV1 while it was stopped in traffic. Photos were taken of the road surface, John's bicycle, and MV2 and submitted to the IT Bureau.

At approximately 1740HRs on Tuesday, July, 23, 2019, I was dispatched to the intersection of Centre St. and Washington St. for a two vehicle crash. Upon arrival, I confirmed that all parties were not injured. The operator of MV1 (MA Pass: 375983) stated that as she traveled northbound on Center St., she stopped at the yield sign in the intersection, so she could enter Washington St. (eastbound). It was at this time MV2 struck MV1 in the rear. The operator of MV2 (MA Official: M94771) stated that as she traveled northbound, she observed MV1 stop at the intersection and began to enter Washington St. MV1 then stopped again and the two collided. I noted damage to MV1's rear bumper area and damage to MV2's front bumper area. Neither vehicle required a tow and left under their own power. I cleared without incident.

On Tuesday, July 23rd 2019, at approximately 5:24pm, I, Officer Brooks, responded to 444 Lowell avenue for a 2 car MVA. Upon my arrival I observed MV1(NH REG 4120041) with heavy front end damage. The vehicle was located in the driveway of 444 Lowell avenue, with the front end sticking out into the roadway. I spoke with the operator of MV1 who stated he was trying to exit the driveway and turn left onto Lowell avenue southbound, when he was struck by MV2(MA REG 3AP243) which had been traveling northbound on Lowell avenue. According to the operator of MV1, MV2 had been traveling at a high rate of speed and tried to swerve around him. There was airbag deployment in MV1 however the operator signed a patient refusal with Cataldo. MV1 was towed by Tody's. I then spoke with the operator of MV2 who stated she was traveling northbound on Lowell avenue and saw MV1 exiting the driveway. She assumed he was going to stop pulling out of the driveway, and when he did not she stated she attempted to swerve around him. MV1 made contact with the passenger side rear wheel well of MV2 causing MV2 to spin out. The operator was not injured and arranged for a private tow due to the damage in the wheel well area.

On 07/24/19 at 08:06 hours, I responded to a two car motor vehicle accident at Dedham and Parker Streets with no reported injuries. Operator of MV1 was stopped at the stop sign on Parker Street (SB) at Dedham Street waiting to take a left turn on to Dedham Street. That is when he was struck in his rear end by MV2. MV1 sustained moderate damage to the rear bumper and hatch area but it was still driveable. Operator of MV2 states she was behind MV1 on Parker Street but did not see that vehicle in front of her. She states she was also attempting to take a left turn on to Dedham Street. She then started to accelerate and struck MV1 in the rear end. I observed moderate damage to the entire front bumper and grille but mainly to the driver's side. The owner decided to have the vehicle privately towed after on Howe Road. Operator was issued MA Uniform Citation # T1445344 for Newton City Ordinance 19-75 Failure to use care in starting and stopping.

Operator of MV1 was traveling southbound on Centre Street approaching the intersection of Pleasant Street. He failed to apply the brakes in time to stop when he saw the operator of MV2 had stopped to allow another vehicle enter onto Centre Street from Pleasant Street. He estimated his speed at the time of the accident to be approximately 10 mph. Operator of MV2 was stopped on Centre Street allowing another driver pull out in front of her when she was struck from behind. Operator of MV2 complained of neck and back pain and was transported to NWH by Cataldo MED1. I observed very minimal damage to both the rear of MV2 and front of MV2. There were a few very minor scratches, but no other damage to either bumpers. The operator of MV1 and his passenger both signed patient refusals. Both vehicles were driveable and operator of MV2 was able to pull her vehicle into a parking spot on Centre Street.

RP David Alessi who is an employee for the chestnut hill mall stated he was alerted to a light post down outside of the mall in the parking lot. David stated he met with operator of MV#1 that stated he was backing up did not see the light poll behind him. He struck the light poll causing it to uprooted and fall over. MV#1 was not on scene I gathered the information from RP David. MV#1 was operating a garbage truck for company JRM whom is subcontracted out with the mall. On 07/25/19 at 10:28 hours, I responded to Centre Street and Boylston St off ramp (WB) for a two car motor vehicle accident with no injuries reported. Operator of MV1 states he was traveling straight ahead on Centre Street (NB) when he observed MV2 inching out of the Boylston Street off ramp. He was not able to stop in time and the two vehicles struck one another. I observed heavy damage to the passenger front bumper and tire area and the vehicle was towed by Tody's. Operator of MV2 states he was exiting the Boylston Street off ramp (WB) and was going to attempt a left turn on to Centre Street after stopping for the stop sign. He then slowly pulled out into the intersection due to National Grid trucks blocking his view and that is when he came in contact with MV1 on his driver's side front bumper. I observed moderate damage to the entire front bumper but the vehicle was still driveable. I did observe National Grid trucks parked at the intersection setting up to do construction work. One truck was parked on the left side of the off ramp and the other was parked on the side where the crash occurred. This did obstruct the operator of MV2's view when pulling out.

At approximately 1620HRs on Thursday, July, 25, 2019, I was dispatched to 444 Washington St. in reference to a truck vs tree motor vehicle crash. Upon arrival, I noted MV1 (MA Vet: 22098) had partially left the roadway and came to rest when it struck a city tree on the sidewalk. Additionally, I noted MV1 struck MV2 (MA Pass: 5CN826), which was parked on the road in front of the tree/aforementioned address. MV1 sustained damage to its front clip and was leaking fluids. MV2 sustained damage to its right rear bumper/quarter panel area. Upon speaking with the operator of MV1, an employee of Tony's Auto Services, he stated that he was test riding the vehicle after they made repairs to its power steering system. He stated that as he traveled eastbound on Watertown St. at approx 30MPH, the vehicle's brakes failed and he guided MV1 towards the tree in an attempt to stop it without injuring anyone. Todys Services took custody of MV1. Newton Fire cleaned up the leaking fluids and the operator of MV1 signed a patient refusal with Newton Medics. Newton Dispatch notified the city to see if they wished to inspect the tree which was still freestanding and appeared to only suffer minor damage. Tony's Auto Services notified the owner of MV1 and I notified the owner of MV2 of the crash. I submitted a picture of the tree to IT to be attached to this report. I cleared without further incident.

The operator of veh #2 stated she was pulling out of Tower Rd attempting to go left (Northbound) on Needham St. and didn't see veh #1 passing as she turned. On 07/29/19 at 11:13 hours, I responded to the Dunkin Donuts parking lot located at 138 Needham Street for a three car motor vehicle accident. Operator of MV1 stated at first she could not recall what occurred during the accident due to lightheadedness at the time. After, she then stated after pulling in to the Dunkin Donuts lot she was attempting to back in to a parking spot in the back of the lot. While backing up, she attempted to hit the brake pedal but must have pushed the gas pedal instead. The vehicle then struck parked/unoccupied MV2 and MV2 was then pushed in to parked/unoccupied MV3. The operator was checked out by Cataldo Ambulance and signed a patient refusal. MV1 sustained moderate driver's side rear bumper area damage but it was still driveable. MV2 sustained damage along both sides of the vehicle and a rear broken axle and it was towed by Tody's. MV3 sustained scrapes and dents to the entire driver's side but the vehicle was driveable.

Operator of Motor Vehicle #1 stated that she was traveling Northbound in the left lane on Dedham Street getting ready to turn left onto Nahanton Street when the car next to her all of a sudden took a left crossing over the double yellow line in an attempt to turn around. Operator of motor Vehicle # 2 stated that she was travelling Westbound on Brookline Street in Newton and wanted to take a left onto Dedham Street however due to the traffic detour she was forced to take a right onto Dedham Street (Northbound). Operator Motor Vehicle # 2 stated that she attempted to take a left turn into a driveway so she could turn around and thought she could make a U Turn instead. Operator of Motor Vehicle #2 stated that she took the left and did not see the vehicle that was in the left lane next to her and struck vehicle # 2. Motor Vehicle #2 sustained moderate drivers side, side damage, and motor vhehicle #1 sustained moderate front end damage. Both parties reported no injuries and signed medical refusal forms.

The operator of MV#1 stated she was in gridlock traffic southbound on Centre St when she struck MV#2 which in turn pushed MV#2 into MV#3. MV#1 sustained minor damages to its front license plate area. MV1 was turning left(east) onto Dexter Rd from Walnut St. As he made the turn he side swiped MV2 who was inching out from Dexter near the center line(not marked). Oper MV1 stated that he was watching MV2 as he turned onto Dexter Rd. He stated that he did not see her stop at the stop sign and she was moving forward at the time of the collision. She was very near the middle of Dexter Rd and was almost past the crosswalk. Operator MV2 stated that she stopped at the stop sign and began inching forward past the crosswalk to see southbound on Walnut St so she could safely turn right(north) onto Walnut St. She did not see MV1 turning onto Dexter Rd. I asked her if she was injured and she stated she was shaken up and she has a past spinal/disk injury. I requested a BLS to evaluate her at the scene(signed refusal) I spoke with witness Gabrielle Wachman 857-204-4144. He was stopped southbound on Walnut St at Dexter St due to traffic. He observed the City truck which was behind him turn left onto Dexter and strike MV2. . He did not see if MV2 stopped at the stop sign. He only saw that she was moving forward past the crosswalk at the time of the MVA. I took pictures of the scene and TOT IT.

OWNER STATES SHE DID NOT SEE WHO HIT HER VEHICLE. AT THE TIME OF THE INCIDENT SHE WAS SHOPPING IN THE SQUARE. OWNER WAS NOT INJURED AS A RESULT OF THE CRASH. OWNER WAS ADVISED TO CONTACT HER INSURANCE COMPANY.

Operator 1 stated he was coming out of Jackson Rd onto Washington St and attempting to turn right onto Adams St. He saw vehicle two sitting in the right turn/straight lane and not moving. cars had begun to proceed through the intersection and go passed vehicle two, so he assumed vehicle 2 was parked or disabled and drove around him on the left in order to make a right hand turn. When he started to turn, vehicle 2 proceeded straight through the intersection striking vehicle 1. Operator of vehicle 2 stated he looked up and saw a green light so he proceeded straight, striking vehicle 1 as it was turning in front of him. there were no injurys and no vehicles were towed.
