Operator 1 stated, she was traveling Northbound on Lake Ave and stopped at the at the stop sign at the intersection of Lake Ave and Beacon St. Operator 1 stated she tried to cross over Beacon St and thought she had enough time to cross but crashed into vehicle 2 passenger side door, who was traveling Westbound on Beacon St. Operator 2 stated, he was traveling Westbound on Beacon St when Vehicle 1 tried to cross the intersection and crashed into his vehicle. A witness stated that vehicle 1 came to a complete stop at the intersection of Lake Ave and Beacon St then proceeded through the intersection and crashed into vehicle 2 at an angle. There was heavy damage to the front of vehicle 1 and heavy damage to the drivers side door and undercarriage of vehicle 2. Both vehicles were towed by Tody's Towing. Operator 1 and her passengers (all juveniles) notified parents/guardians on scene about the accident and all declined medical attention. Operator 2 also declined medical attention.

Operator of MV1 said she was on the on ramp located at Winchester Street at Boylston Street. Operator of MV1 said she attempted to merge onto Boylston Street when she struck the rear of MV2 at Boylston Street at Walnut Street. Operator of MV2 said he was traveling eastbound on Boylston St and was attempting to turn right onto Walnut St when he was struck from behind by MV2. Both MV1 and MV2 pulled over to Woodcliff road. There is damage to the rear passenger bumper of MV2. There were no injuries and both vehicles were able to drive away from the scene.

Operator of vehicle 1 states she was traveling northbound on Hammond St crossing Commonwealth Ave and had a green light. As she was crossing Commonwealth Ave vehicle 2 came across the front of her vehicle from her right (traveling westbound) and the vehicles collided. Operator of vehicle 2 stated she was traveling westbound on Commonwealth Ave and came to the intersection of Hammond St. Operator stated she had no idea if the light was green or red due to solar glare but continued through the intersection where she was struck on the drivers side by vehicle 1. Both operators and the two passengers of vehicle 1 signed patient refusals with Fallon medics. Vehicle 1 was able to be driven from the scene. Vehicle 2 was towed by Todys. The reporting party to the accident was an uninvolved witness. The witness told dispatch that vehicle 2 ran the red light causing the accident. I attempted to follow up with the witness by calling the number they called from (617-462-6762) with negative results. I did leave a voice mail and will attempt to follow up on my next tour of duty. ***On 10/07/2021 I was able to speak to the witness of the accident, Caroline O'Brien. O'Brien states she was on Hammond St behind vehicle 1 and states that they had the green light to proceed. She also states she saw vehicle 2 traveling westbound on Commonwealth Ave at a high rate of speed run the red light and strike vehicle 1.

MV1 was travelling North bound on Parker St. while waiting in traffic MV2 had rear ended MV1. Operator of MV2 exited vehicle to assess damage, then proceeded to get back in his vehicle and leave. Owner and passenger of MV1 proceeded to step in front of MV2 to obtain his information, but MV2 proceeded to drive towards the operator of MV1. Passenger of MV2 stated he placed his hands on the hood of MV2's vehicle to block him from leaving. MV2 proceeded to drive around MV1 passenger and left the scene. The driver and passenger of MV1 described the operator of MV2 as an older black male, in his 70's, with a salt and pepper beard and short black hair and wearing a white t-shirt. Driver and passenger of MV1 are unsure if the operator of MV2 is under the influence, due to the operator appearing "dirty" and having "eyes really wide open." It was asked if they could smell alcohol on the man or if he walked unsteadily, which the reporting party stated "no." Driver and passenger of MV1 stated they were okay and did not sustain any injuries and declining medical attention. MV1 had minimal damage to the rear bumper consisting of scratches. A description of MV2 was given as a gray Honda Civic with a Connecticut license plate. Upon clearing the scene, I canvassed the area yielding negative results. State Police was also notified, due to the accidents close proximity to Route 9.

On 10/7/21 at about 1145am I was dispatched to 1101 Beacon St the Walgreen's parking lot for an MVA with injuries. On arrival, I observed 3 parked unoccupied MVs heavily damaged that were smashed into each other. I was approached by one of the MV owners, Tony Dooley. Mr Dooley stated he was walking out of the Walgreen's front door heading to his parked MV (#3) which was approximately <30 feet away. His attention was suddenly drawn to a speeding MV (MA reg 3MKH84) driving through the lot that veered right and smashed into the 3 cars. Mr Dooley's MV was the middle car smashed between the other 2. The female operator briefly stopped afterwards and pulled into an adjacent parking spot at a crooked angle. I approached the female operator who was being taken out of her car and placed onto the stretcher by medics for transport. Female operator was identified as Ms Norma Greenberg. I asked her what happened? She replied in a bewildered manner "I have no idea." I asked her if she was taking any medications today and she replied yes, to treat a spinal condition she has. Ms Greenberg had lacerations on her legs and arm with heavy bleeding as a result of the impact.Ms Greenberg's vehicle (#1) was towed with significant front damage and airbag deployment. MV#2, the first car struck on impact, was towed with very heavy passenger side, rear end damage and driver's side damage. MV#3, the middle vehicle, was towed with significant passenger side damage, flat tire and driver's side damage. MV#4, sustained significant passenger side damage which was able to be driven from the scene. Based on observations and statements, it appeared Ms Greenberg had suddenly dangerously accelerated through a congested parking lot causing heavy damage. Ms Greenberg's driving history shows surchargable accidents from 7/1/21, 11/1/20, 8/25/15 and 12/26/10. Due to Ms Greenberg's inability to recall how or what happened, I am seeking an Immediate Threat application due to and not limited to medical reasons to suspend Ms Greenberg's license through the RMV.

The operator of MV1 stated that they were at the stop sign on Webster St. and took a left onto Elm heading Southbound. Once on Elm St. MV2 which was parked on the right side of the street, pulled out and hit MV1 on the right side. MV1 had moderate damage to the right side of the truck, and a rear flat tire. The operator of MV2 stated that they were parked on Elm St. and when they went to pull out MV1 crashed into them. The operator of MV2 stated that they checked for incoming traffic and did not see MV1 coming down Elm St. There was major damage to the front of MV 2. The operator of MV1 was able to change the flat tire and take the vehcile, Todys took possesion of MV2. Both parties denied medical attention, and were given the report number.

On 10/13/2021 at approx 0803Hrs while assigned to 497 I responded to the area of 349 Auburn St for a report of a two car crash with injury. Upon arrival I observed Ma Reg 1FLA38 disabled in the middle of the road with heavy front passenger side damage, with both front and passenger side airbags deployed. Fallon ambulance was on scene with the operator , John McHugh on the stretcher with a head injury. The Fallon EMT's Alex Honrado and Steve Hough stated they had been parked on Auburn St when McHugh ran into the back of a parked landscape trailer. I spoke with McHugh who stated he could not see out his windshield as he had just left his home on Bourne St and his windshield was not defrosted yet and ran into the trailer. McHugh and Fallon Ambulance personnel said as there had been no damage to the trailer the landscaper had left and nobody got a plate number or company name. McHugh transported to NWH, Todys towed the vehicle. A routine registry check showed McHugh has a suspended license.Ma Uniform Citation T2015320 for criminal application for violation of MGL Chec90 Sec 23 Operating After Suspension mailed to McHugh.
