I responded to the area of Washington St.@Quinobequin Rd. for a report of a two car MVA. I arrived in the area and located the MVA directly at the entrance to Quinobequin Rd. Both Operators and passenger were out of the vehicle upon my arrival with no reports of injury. V1 Operator; Cunniff, Erin MA OLN#S65270968. V1; MA Pan Reg. 6PW389, 2011 SUBA/STW color gray. V2 Operator; Murphy, Kelly RI OLN#3262011. V2; RI Pan Reg. 410986, 2011 Suzu/SX4 color red. V2 Passenger; Revoy, Antoine. V1 operating Eastbound on RT. 16 Washington St. attempted to turn right onto Quinobequin Rd. The operator of V1 traveled past the entrance, came to a stop and attempted to backup. V1 while reversing backed into V2 which was also traveling Eastbound on RT. 16 Washington St. and was attempting to turn right onto Quinobequin Rd. The V2 Operator and Passenger both stated that V2 Operator beeped its horn but V1 backed into V2 On Washington St., at the entrance to Quinobequin Rd. There was no air bag deployment in either vehicle. V1 sustained right rear end damage which did not require towing service. V2 sustained heavy left front end damage and left side damages to include the tire and wheel well. V2 did require towing service. Tody's Towing Inc. responded and removed V2 from the crash scene. I advised all parties involved. I also transported V2 Operator and Passenger To Tody's Towing located at 1354 Washington St., Newton.

On 01/11/2019, I responded to 1466 Chestnut St for a four vehicle accident. On arrival, I spoke with the operator of MV1 who stated that she was driving westbound on Commonwealth Ave and was unsure what happened but thought the vehicle in front of her came to a sudden stop. MV1 stated she attempted to stop but could not, and collided with the rear of MV2. The operator of MV2 stated that he was stopped on Commonwealth Ave behind MV3 when he was hit from behind by MV1 and pushed into the rear of MV3. The operator of MV3 stated she was stopped behind MV4 when he was rear ended by MV3, and pushed into the rear of MV4. The operator of MV4 stated that she was stopped on Commonwealth Ave due to traffic turning into the driveway of 1466 Commonwealth Ave. MV4 stated that she was stopped, with her left turn signal on for some time before she heard the sounds of a car accident behind her. MV1 sustained heavy front end damage and was towed by Tody's. MV2 sustained rear end and front end damage and was towed by AAA. MV3 sustained rear damage and minor front end damage. MV4 sustained minor scratches to the rear bumper. All parties involved declined medical attention.

Operator of MV1 stated he was traveling Eastbound on Commonwealth ave in his lane of travel when he observed MV2 parked on the right hand portion on Commonwealth ave. Operator of MV1 stated as he was approaching MV2 he attempted to slow down in an effort to avoid colliding with MV2, but stated he could not stop due to the slippery surfaces on the road and collided with MV2. MV2 was parked and unoccupied at the time of the accident. I spoke to Owner of MV2 who shorty arrived on scene after the accident who stated he parked in front of 676 Commonwealth ave for a while to shovel snow in his driveway. No injuries. MV1 was towed by AAA and MV2 was driven off by owner to his nearby residence. MV1 sustained heavy right front end damage. MV2 sustained heavy rear end damage.

MV1 stated he was "driving to fast around the corner and hit the pole." MV1 actually hit a curb, took down a street light, went up on the island hitting another street light prior to hitting the curb. The second light he struck fell on MV2. MV1 sustained heavy front end damage and was removed from the scene by Tody's Towing. MV2 stated that MV1 "came flying across the intersection, crashed into 2 poles, one of which hit my car and he crashed over there." I then observed minor damage to MV2, however it did not require a tow. Both parties signed patient refusals with Cataldo Ambulance. Operator of MV1 was arrested and charged with the following: M.G.L. 90/24/J OUI Liquor M.G.L. 90//24/E Negligent Operation M.G.L. 89/4A Marked Lanes Photos of the scene were captured and turned over to the IT Bureau. Daigle Electric notified of the damaged lights.

The operator of MV1 stated she was traveling Westbound on Washington St. at Church St. in the left travel lane and was coming to a stop behind several other cars at the red light. She stated as the light turned green she was struck from behind by MV2. As she proceeded to pull over, MV2 continued to travel Westbound on Washington St. and didn't stop. MV1 sustained minor damage to the right rear and was able to drive away from the scene safely. MV2 came back registered to GOMES, Leonel of 72 High St. Waltham, and dispatch requested Waltham PD respond to check for the vehicle. Waltham PD located the vehicle in the driveway and the registered owner was sitting in the vehicle. An ELMUS, Elmer was on scene to translate for the operator as he only spoke Spanish. Dispatch relayed to me that there was no status found on GOMES' license, and upon speaking to GOMES and ELMUS they confirmed he did not have a license or passport. GOMES relayed to ELMUS that he was aware that he struck MV1 at the intersection of Washington St. at Church St. He stated that the light was green and that he was traveling Westbound on Washington St behind MV1. He attempted to brake and turned his wheel to the right to avoid the crash, but struck MV1 in the right rear. He stated that he thought MV1 was going to leave so he didn't stop and drove back to his residence in Waltham. MV2 sustained minor damage to the front left bumper. All parties involved were uninjured.

On 02/06/2019, at 1800 hrs, I responded to the area of 787 Washington St for a two car MVA with possible minor injuries. Upon arrival, I observed MV1 and MV2 both facing eastbound on the center double yellow line. It appeared that MV1 was in the outer lane and changed lanes, striking MV2 in the inner lane. MV1 had damage to the drivers side and MV2 had damage to the passenger side. The operator of MV1 stated he changed lanes and didn't see MV2 before they collided. The operator of MV2 stated MV2 abruptly changed into her lane and struck her. A witness corroborated the above statements. MV2 was towed by Tody's and both parties involved signed patient refusals with Cataldo.

On 02/08/2019, I, Officer Conary, responded to 309 Tremont Street for a past hit and run involving a pedestrian. Upon arrival, I was met by FEBLOWITZ, Colleen, who stated at approximately 1745 hours, she was crossing in the crosswalk at Tremont at Hibbard when her foot was hit by a car. FEBLOWITZ explained to me that she was in the crosswalk and she saw a silver sedan, MV1, traveling Eastbound on Tremont Street approaching her but it was far away where she felt safe to walk across the street. She said she also raised her hand to draw attention to herself. FEBLOWITZ was halfway in the crosswalk when she felt severe pain in her right foot. She looked behind her and saw MV1 was very close to her. MV1 continued to drive down Tremont Street and not stop. There were no witnesses. FEBLOWITZ went to Partner's Urgent Care to be treated for minor injuries. The crosswalk is in a lighted roadway. FEBLOWITZ was unable to gain any other information on MV1. No further incident to report.

Operator 1 states he was traveling Eastbound on Beacon St and came to a complete stop for pedestrians in the crosswalk. While he was waiting he was struck from behind by vehicle 2. Operator 2 stated he was traveling Eastbound on Beacon St and didn't see vehicle 1 stop in front of him and he struck vehicle 1. MV 1 sustained minor damage and vehicle 2 sustained moderate damage and was towed by Tody's. There were no injuries.

MV1 stated he "had his directional on, was trying to get into the right lane" however MV2 "wouldn't let me." MV1 further stated when he did get behind MV2 she "drove across all the lanes and would hit the gas & brake, so when i tried to pass her she slammed into my car, then pulled in front of me, slammed on the brakes so I'd hit her, however I don't think I did." The operator of MV1 further stated that the operator of MV2 then exited her car began filming him while shouting "you hit me, then drove off." MV2 is described as a "black ford or subaru sedan driven by a white female in her 20's, with blond hair & glasses." MV1 stated he was unable to get a license plate. A canvass of the area was conducted yielding negative results.

The operator of MV#1 stated that she had just made a U-Turn from the east bound side of Commonwealth Ave (#2345) to the west bound side when her MV started to slide in the fresh snow. MV then slid across Commonwealth Ave and hit the curb and then head on into a large tree. Operator arranged for their own tow.

Oper # 1 stated he was driving on Washington Street on 1/30/19 at 20:00 and spun out causing him to hit a pole. Claims no damage to pole and minor damage to car. Only reporting to satisfying Insurance company.

The operator of MV1 (ma con T98504) was traveling WB on Washington Street attempting to take a left into 300 Washington Street when his MV was rear ended. Operator of MV stated he was letting a vehicle exit from 300 Washington Street and had come to a complete stop when his vehicle was struck from behind. The operator of MV2 (ma con T12376) stated he was traveling WB on Washington Street and was unable to come to a complete stop before striking the rear of MV1. Air bags were deployed in MV2. The operator of MV2 stated he felt fine. Ambulance two and Ladder 3 were dispatched to the scene to evaluate the operator. Operator of MV2 signed a refusal. The other parties involved were uninjured. Tody's was also called on scene to tow MV2 as it sustained heavy front end damage and was not drive able. MV1 sustained minor damage to rear door and bumper but was able to operator without any issues.

Operator 1 states she was traveling Northbound on Chestnut St attempting to make a left onto Wyman St. The vehicle in front of vehicle 1 was also turning onto Wyman St; however, it stopped suddenly due to a tree down on Wyman street. Vehicle 1 stopped and then was stuck from behind by vehicle 2. Operator 2 stated that she was traveling on Chestnut St behind vehicle 1. Vehicle 1 started to turn left but came to an abrupt stop. Operator 2 did not have time to stop and struck vehicle 1 from behind. Vehicle 1 sustained minor damage and vehicle 2 sustained moderate damage and was towed from the scene by Tody's. There were no injuries.

On 02/26/2019, at 1852 hrs, I responded to the intersection of Washington St and Walnut St for a four car MVA. Upon arrival, I observed all four vehicles in the inner westbound lane of Washington St. All four vehicles sustained damage from a chain reaction rear end accident caused by MV1 in the rear. MV1 sustained heavy front end damage and was towed by Tody's. MV2 and MV3 sustained damage to their front and rear ends. MV4 sustained damage to its rear end. I spoke with the driver of MV1 who stated that he was distraught about family issues and looked away from the road before rear ending MV2. The other three drivers all stated that they were slowing or stopped at the red light when they were rear ended, corroborating the driver of MV1's statement. No injuries were reported and Cataldo received patient refusals.

MV#1 stated while traveling westbound on Commonwealth Ave he closed his eyes for a few moments causing his vehicle to veer to the right. MV#1 drove up on the curb and struck a overhanging street light. MV#1 sustained heavy front end damage as well as uprooting the overhanging street light. Operator of MV#1 stated he had minor injuries and signed a patient refusal. Operator of MV#1 was given the following roadside tests, due to the time and circumstances of the incident. Horizontal Gaze Nystagmus - Pass One Leg Stand - Pass Walk And Turn - Pass Operator of MV#1 showed no signs of impairment. Through my training and experience I calculated MV#1 had speeds of approximately +50MPH. Tody's towed the MV, a towed MV inventory form was completed. Pictures taken on scene submitted to IT.

On Saturday March 9, 2019, at approximately 0230 hours, I, Officer Brandon, responded to the Weston/Newton line on Commonwealth Ave to assist Weston PD with field sobriety. On arrival Weston PD had concluded field sobriety and determined that the party, Glenn Matot, was not under the influence. Weston PD then explained that they had been contacted by AAA stating that a single vehicle accident occurred in Weston's jurisdiction. After further review, Weston PD and I determined that the accident was in fact in Newton. I then spoke with the operator of MV1, Glenn Matot, who stated that he was on his way home from work traveling westbound on Commonwealth ave in front of the Charles River Canoe and Kayak when he became distracted by a text message. Mr. Matot further explained that while texting, his car drifted to the right causing him to drive up on the snow bank and up onto the guard rail before then falling off back into the road. MV1 sustained major damage to both the passenger side and undercarriage. Mr. Matot did not sustain any injuries and denied any medical attention. AAA arrived on scene and removed the vehicle from the road and Mr. Matot took an Uber home. Two pictures were taken of the damaged City of Newton guard rail and submitted to IT to be attached to the report.

The operator of MV#1 stated that she was traveling south on Washington St in the left hand lane when her MV was struck by MV#2. The operator of MV#2 stated that he was also traveling south on Washington St in the right hand lane directly next to MV#1 when his MV began to slide due to icy road conditions and his MV struck MV#1. No injuries & no citations

The OP. of MV#1 states she had gotten off Rt-128N and was stopped waiting to merge onto Washington Street eastbound. She was then suddenly rear ended by MV#2. The OP. of MV#2 states he was initially behind MV#1 and thought MV#1 had already merged into traffic. When he saw a gap in traffic on Washington Street, he proceeded forward and collided into the rear of MV#1. I observed very minor damage, paint transfer and scratches. The OP. of MV#1 signed a refusal with the medics and will seek her own medical attention.

Operator of vehicle #1 stated he was travelling Westbound on Crafts St. and stated he became distracted by a noise coming from the left front wheel or axel. Operator #1 stated he took his eyes off the road and drifted to the right striking a utility pole. Operator # 1 had only a Guatemala ID card. Operator # 1 was cited for Unlicensed operation citation #T1268256.

Operator of Vehicle #1 stated that he was traveling North on Centre Street. He passed through the intersection of Cabot and Centre street where he proceeded downhill on an icy road. Operator stated that he "fishtailed" and lost control of his vehicle before leaving the roadway and coming to rest in the shrubs and trees in the front yard of 700 Centre Street. At the time of this incident there is a reported water main break at the intersection of Centre street and Sargent street with City Water Crews addressing the issue. I observed a large collection of ice in the intersection and in the northbound lane of Centre street. No injuries were reported. Tody's was notified and removed the truck from the bushes. Operator #1 drove the vehicle away. I notified the home owner, Mr. Watkins of the damage. He estimated the damage to the landscaping to be approximately $2000.

MV1 stated she was stopped on Washington St attempting to turn left onto Church St when she was rear ended by MV2 causing minor damage. MV1 did not require a tow, was evaluated and cleared by Cataldo Ambulance and departed the scene. MV2 stated she was "trying to get in the right lane but couldn't and when I came back I hit that car" causing minor damage. The operator of MV2 complained of pain to her head from striking the windshield and was transported by Cataldo to Newton Wellesley Hospital for an evaluation. Her vehicle was left parked legally on Washington St.

The operator of vehicle 1 stated she was turning right onto Washington Street from the I95N off ramp when she was rear-ended, this caused minor damage to the bumper and exhaust of her vehicle. She did not get a good look at the other vehicle and it did not stop.

On Thursday, April 4th 2019, I responded to a one vehicle accident on Dedham St by Oak Hill St. The vehicle (Mass Commerical # 704 ADVA MIXER Color Red) was operated by Carlos Paz. His vehicle was headed west on Dedham St when the top of his vehicle struck a low hanging wire and dragged other wires to the point where it made utility pole # 104 tilt over toward the middle of the rd. There were no injuries. Eversource and Verizon responded to fix the pole. Carlos suffered no damage to his vehicle. Carlos has Travelers Insurance.

On 04/08/2019, at 1620 hrs, I responded to the area of 2040 Commonwealth Ave for a three car MVA with injuries. Upon arrival, I observed all three vehicles facing westbound on Commonwealth Ave. MV1 had heavy front end damage and airbag deployment. MV2 also had heavy front end damage and airbag deployment. MV3 had minor rear end damage. The operator of MV1 was brought to Newton Wellesley Hospital by Cataldo medics. The operator of MV2 stated she was on Pamela Ct attempting to take a left onto Commonwealth Ave. There was heavy traffic headed westbound and a vehicle stopped to let MV2 go. As MV2 was taking a left onto Commonwealth Ave, MV1 was traveling westbound in the eastbound lane around traffic and struck MV2. MV2 then in turn struck MV3s rear end. The operator of MV3 stated that he wasn't sure what occurred because the accident was behind him. The operator of MV1 stated she was traveling westbound on Commonwealth Ave and was planning on taking a left into the Star Market parking lot which was approximately 250 feet down the street. She stated she thought there were two lanes in the westbound direction at that point in the roadway. MV2 then pulled out in front of her and they collided. Two witnesses who were traveling westbound in traffic a few vehicles behind MV3 both stated that MV1 drove erratically into the oncoming lane and around traffic heading westbound. One witness stated "the car was almost completely in the opposite lane" and the other witness agreed. A moment later they stated they heard the crash but did not see it. They also stated that she almost crashed into a car turning eastbound out of the Star Market exit prior to the accident. MV1 and MV2 were towed from the scene by Tody's. The operator of MV2 was brought to Newton Wellesley Hospital by her mother. The operator of MV1 was cited for C90 S23 operating after revoked license, C90 S23 operating after revoked registration, C90 S24 operating to endanger, and C89 S4a marked lanes violation. All aforementioned streets are public ways maintained by the City of Newton.

Operator of MV1 stated while traveling northbound on Centre St. vehicles in front of her came to a halt. As she came to a stop, MV2 struck her rear driver side. Operator of MV1 stated MV2 wasn't traveling too closely but due to the vehicles infront of her stopping quickly she had to stop quickly aswell. Operator of MV2 stated while traveling northbound on Centre St, just passed the intersection of Centre St. and Walnut St. MV1 came to a stop. Operator of MV2 stated he attempted to pull onto the sidewalk to avoid rear ending MV1 but did not have enough room and struck MV2. MV1 had significant damage to the rear passenger side. Operator of MV1 was able to drive her vehicle home. MV2 had significant damage to the front driver side and driverside door. MV2 was not able to operate the vehicle due to the damage. Todys's towed the vehicle.

Operator of MV1 states he was traveling eastbound on Nahanton Street approaching Winchester Street when he entered through the intersection and struck a deer that unexpectedly appeared from the woods crossing the roadway. MV1 sustained minor damage to the front right bumper and headlight.

Operator of MV1 (ma reg 85CD87) stated he was traveling EB on Washington Street in the left lane when he was struck from a motor vehicle attempting to turn onto the WB side of Washington Street. Operator of MV1 stated he was traveling straight on Washington Street when a motor vehicle attempted to cross over Washington Street from the RT 95 exit ramp. MV1 was struck on the passenger's side door. Operator of MV2 stated she was stopped on the exit ramp of RT 95 and Washington Street attempting to turn left onto Washington Street. Operator of MV2 stated the first lane of traffic on the EB side of Washington Street was stopped when she edged out. Operator of MV2 stated she thought the other lane was clear when she attempted to turn left onto Washington Street. Operator of MV2 stated that as she started to enter into the second lane MV1 was approaching and her front bumper struck his passenger side door. Both drivers stated they were not injured and required no medical treatment at this time. Both vehicles suffered damage but were able to drive away from the scene without any issues.

MV#1 was parked outside 719 Washington St (was not occupied at the time) when MV#2 attempted to park in front of MV#1. MV#2 side swiped MV#1 on the front driver side causing moderate damage to MV#1. MV#1 was Towed at the request of the owner, Tody's took posetion of the vehicle. MV#2 stated he did not sustain any injuries at this time.

The operator of Vehicle #1 stated as she was travelliong eastbound on Washington St, Vehicle #2 collided with her vehicle as it crossed into her travel lane and slammed on his brakes, leaving her without time to stop. The resulting collision caused damage to the front of the vehicle, a well as the quarter panel. Vehicle #2 then fled the area by driving through the parking lot, and back onto Washington St, in the opposite direction. 2 witnesses, identified as Ms. Christine Cole. and Mr. Ed Cronin, gave a similar account of the incident. Neither party was able to get a plate. The only description is of a white SUV type vehicle. A canvass of the area yielded negative results.

The operator of Vehicle #1, identified as Ms.Erzsebet Piros, stated that she took a left turn from Walnut St onto Washington St prior to the colliding with Ms. Kim Nguyen. She further stated that she did not see the pedestrian prior to hearing the collision. Her passenger, identified as Ms. Amy Torres, was sitting in the front passenger seat. She stated a similar account, and did not see the pedestrian prior to the collision. Neither party complained of injury, when asked. A witness, Ms. John Angelucci, was travelling in his vehicle, in the lane next to Ms. Piros. He stated that he only heard the collision, and couldn't see it due to vehicle lights, and the falling rain. The pedestrian, Ms. Kim Nguyen, stated that she attempted to cross the street, when she did not see any cars coming. She stated that she didn't see Vehicle #1 approaching until it was too late to get out of the way. It should be noted that Ms. Nguyen was not attempting to cross inside of a crosswalk, and was wearing dark colored/Navy Blue clothing. Ms. Nguyen was transported to Beth-Israel Hospital, and was communicating effectively prior to transport. Pictures of the scene, and damage to the vehicle were taken, and sent to IT for entry.

MV1 operator states he was driving around the Newton Corner Rotary and approached the merge at the beginning of the bridge to change lanes to head towards Waltham/Watertown, when MV2 collided with his vehicle. Damage was observed on the front passenger door. No injuries reported. MV2 operator states she was driving on Washington St and approached the merge at the beginning of the bridge to change lanes and head onto the Mass Pike W-Bound, when MV1 collided with her vehicle. Damage was observed on the front drivers side bumper. MV2 operator reported having whiplash but signed a medical refusal and was not transported to the hospital. I then cleared without further incident. The main crash factor was due to the weather being a heavy rain storm at night. It should be noted there is a yield sign facing MV1s incoming direction.

The operator of Vehicle #1 stated that as he was attempting to make a left turn from Washington St, to Lewis St, he collided with Vehicle #2. He added that he believed Vehicle #2 was travelling at a high rate of speed. The operator of Vehicle #2 stated that as he was travelling Eastbound on Washington St, he collided with Vehicle #1 as he attempted to make a left turn. The operator of Vehicle #3 stated that Vehicle #2 was travelling at a high rate of speed, and went through the red light, prior to the collision with Vehicle #1. As a result of the collision with Vehicle #2, Vehicle #1 struck Vehicle #3 with its rear driver's side. Vehicle #3 was damaged in the front bumper, as well as the driver's side quarter panel. The operator of Vehicle #1 was transported to Newton-Wellesley Hospital for an evaluation. Both Vehicles 1, and 2 were towed by Tody's Towing Co.

On 04/27/2019, while assigned to N494, I, Officer Conary, responded to Commonwealth Ave at Lowell Ave for a MVA. Upon arrival, I observed two sedans in the middle of Commonwealth, facing opposite directions. I told both operators to pull to the side of the road. Operator of MV1 stated that he was traveling Southbound on Lowell Ave and took a left turn onto Commonwealth Ave going Eastbound. When he took the turn, he said he took it too sharp and got distracted with a lip balm in his car. He looked down because it fell on the side of the driver's side door. When he looked down he entered the Westbound lane of traffic and hit MV2. Operator of MV2 stated that she was traveling Westbound on Commonwealth Ave when she saw MV1 swerving into her lane. She attempted to avoid MV2 hitting her, but MV1 hit her head on. Both operators were offered and declined medical attention. Both vehicles were driven from scene. No further incident to report.

Vehicle one was stopped in traffic when vehicle two rear ended vehicle one. Vehicle two stated she looked down at the radio quick and then rear ended vehicle one. vehicle one was transporting two male parties while working for LYFT. Both male parties of vehicle one were transported to Newton Wellesley Hospital.

Bicyclist was traveling west on Beacon St coming down the hill towards Hobart Rd. He stated that he started braking when his light turned yellow and his brakes malfunctioned and he could not stop before Hobart Rd traffic got a green light. He struck the rear quarter of a red Toyota SUV that came out of Hobart Rd. The MV did stop and check on the bicyclist. The operator took out a video camera and recorded the bicyclist admit that the MVA was his fault. They did not exchange information and the operator stated that he could not wait for police to arrive. The bicyclist signed a medical refusal. Pictures were taken by me of the damaged bicycle. According to the bicyclist, there was no damage to the MV.

I responded to the area of Commonwealth Ave@Lexington st at approx 1610 hrs on 05/09/2019 for a report of a H&R/MVA. I arrived in the area and located Sarah Hamilton. Sarah informed me that she was on Comm. Ave (westbound) stopped attempting to turn left into the Dunkin Donuts parking lot. Sarah stated she was rear ended by a silver sedan operated by a female operator. Sarah spoke with the unidentified woman, who refused to exchange information. The woman returned to her vehicle and left the area. Sarah could not get the license plate, vehicle make /model. Sarah Hamilton complained of neck pain. EMS responded to medically evauate her. Sarah did not want to be transported to the hospital and signed a refusal/waiver with EMS. The only damage I observed to V1 was paint scratches along the rear bumper. The rear bumper was intact free of any dents/creases.

The operator of vehicle 1 MA REG: 1AB918 stated she was traveling West on Washington Street and did not see vehicle 2 MA REG: 925WP2 at all until she crashed into the rear driver side of it. This caused major damage to vehicle 1 and minor damage to vehicle 2. Both drivers exchanged information and the operator of vehicle 2 left the area. The operator of vehicle 1 remained there to speak with police and wait for a private tow company.

MV1 operator states he was driving Eastbound on Washington St. passing the Rt. 128 off ramps when MV2 collided with his vehicle. No injuries reported on scene. MV1 was able to drive his vehicle to Belmore Pk. where he will wait for AAA. I observed damage to the right side and rear right side of the vehicle. MV2 operator states he come up to the stop sign at the Rt. 128 off ramp, stopped and then proceeded when clear. As MV2 operator was crossing over the lanes to take a left hand turn to go Westbound on Washington St., he did not see MV1 in lane 1. At this point, MV2 collided with MV1. No injuries reported on scene. MV2 was able to operate his vehicle. I observed damage to the front and front right side of the vehicle. I then cleared without further incident.

MV1 stated he was travelling eastbound on Washington St when MV2 "pulled out in front of me" causing front end damage. MV2 stated he was attempting to pull onto Washington St and the "first car let me go, and that guy was speeding and crashed into me" causing front end damage. No parties required medical attention and both vehicles were removed from the scene by Tody's. Operator of MV2 was cited under 89/8 Failure to Yield.

Operator 1 states he was traveling Eastbound on Beacon St when vehicle 2 drove through the stop sign on Chestnut Hill Dr and into the intersection. Operator 1 did not have time to stop and struck vehicle 2. Operator 2 stated he was stopped on Chestnut Hill Dr. at the stop sign. Operator 2 thought he had enough time to cross Beacon St and proceeded forward. Operator 2 was struck by vehicle 1 in the intersection. Vehicle 1 and 2 sustained moderate damage and were towed. There were no injuries.

MV1 stated he was traveling straight when he was "rear ended" by MV2 causing minor damage to the rear bumper. MV2 stated she was travelling straight when MV1 "cut in front of me and hit me" causing minor front end damage. No injuries reported and no tows required. With no witnesses or cameras, I am unable to determine who is at fault.

Bicyclist was traveling north on Grant Ave with traffic on the right side of the cars. He was intending on traveling straight across Comm Ave. The unknown silver BMW who was traveling next to the bicyclist turned right onto Comm Ave eastbound and struck the bicyclist with rear right part of the car. The BMW continued east on Comm Ave. The bicyclist was knocked from his bike and sustained minor cuts to his legs. His bike was not damaged. He refused medical attention and left the scene prior to amb arrival. No pictures were taken because he left before I could take them.

I responded to the area of Washington St@Belmore PK. at 1710hrs. 05/28/2019 for a two car MVA. I arrived on location and identified both operators. V1 operator; Louis, Kevin K. MA OLN #S43050796 operating MA Reg. 4RT219; 2003 BMW/X5 color black. V2 Operator; Whalen, Holly J. MA OLN#S25300910 operating MA Reg. 23XR36; 2008 Chev/Suburb color blue. Both vehicles were traveling westbound on Washington St. V2 slowed/stopped for a line of traffic in front of her. V1 ,as stated by the operator, was changing lanes when V2 came to a stop and collided into the rear of V2. Neither operator was injured as a result of the collision. V1 did have airbag deployment. V1 operator declined EMS. V1 required towing service. Tody's responded to provide service. V1 sustained front end damage. V2 sustained minor damage to the rear bumper/body. V2 did not require towing service.

MV 1 was traveling straight going west on Washington Street. MV 2 proceeded to merge into traffic lane from a parked spot. MV 2 did not see MV 1. MV 2 collided into MV 1. MV 1 was not speeding and had the right of way. Minor damage to both vehicles and no injuries.

On June 24, 2019 at approximately 17:12 hours I responded to a two car motor vehicle crash on Washington St at the intersection of Prospect St. Both vehicles were pulled over facing westbound on Washington St. MV1 had rear end damage, while MV 2 Had front end damage. Both operators said that the vehicle in front of MV1 made a sudden stop and turned left onto Prospect St without signaling, MV1 had to brake suddenly and MV2 crashed into the rear of MV1. The operator of MV1 complained of back pain but declined transport to the hospital after evaluation. MV2 was towed from the scene by Tody's.

Operator of Vehicle # 1 states that he was traveling straight ahead (NB) at 148 Parker Street when he stopped for approximately five children who signaled the cross walk signal to cross there. While completely stopped, he was rear ended by Vehicle # 2 and his vehicle was pushed forward approximately ten feet. His vehicle sustained major rear end damage and it was towed by Tody's. There is a functioning crosswalk signal there which has overhead lights that flash yellow before turning red. Operator of Vehicle # 2 states she was traveling straight ahead (NB) on Parker Street when she observed the above mentioned children on the side of the road. Next she says that she saw the crosswalk lights above flashing yellow but never saw Vehicle # 1 stopped there until the last second. She was then not able to stop in time striking # 1 in the rear end. Her vehicle sustained heavy front end damage and it was towed by Tody's. The operator complained of neck pain but signed a patient refusal with Cataldo.

Vehicle 1 was traveling west on Washington St. Vehicle 1 wanted to make a left turn onto Day St. but went past the street. Vehicle 1 reversed traveling backwards in the westbound travel lane. At this time vehicle 1 reversed into vehicle 2. Both operators of both vehicle 1 and 2 state the same story. No reported injuries. Minor damage to the right rear of vehicle 1 and the left front of vehicle 2, both vehicles driveable.

Operator of Vehicle #1 stated that she was traveling on Nahanton street and stopped to let a golfer cross the street from one side of the country club to the other. Vehicle #1 was rear ended by Vehicle #2 and sustained heavy rear end damage. Vehicle #1 was towed from the scene by David from Tody's. The operator of Vehicle #2 stated that he was traveling on Nahanton Street when a golfer ran across the street in front of Vehicle #1. Operator stated that he couldn't stop and struck Vehicle #1. Vehicle #2 sustained front end damage and was towed from the scene by Bobby from Todys. Both operators were picked up from the scene. No injuries were reported. It should be noted that there is no crosswalk at the location of the crash.

MV1 stated she "was working all day and closed her eyes for a moment" subsequently rear ending MV2 which was stopped in traffic. MV1 sustained heavy front end damage and was removed from the scene by Tody's. MV2 stated he was stopped and he could see MV1 "in my rear view mirror coming up fast without hitting the brakes" subsequently rear ending his vehicle. MV2 was then pushed forward striking MV3. MV2 sustained minor front and rear end damage. MV3 stated she was at a stop and was rear ended by MV2 and as a result she rear ended MV4. MV3 sustained minor front and rear end damage. MV4 confirmed that she was stopped at the light and was rear ended by MV3. MV4 sustained minor rear end damage.

(cid:31)Operator of vehicle #1 was travelling Westbound on Washington proceeding through the intersection at Washington And Adams St. Operator #1 stated she observed vehicle #2 which was heading Eastbound on Washington St. attempt to make a left turn onto Adams St. in front of her. Operator attempted to stop but was unable to in time and collided with vehicle #2. Operator of vehicle #2 stated he was travelling Eastbound on Washington St. and had a green light. Operator #2 attempted to make a left turn onto Adams St. in front of vehicle #1 and collided with vehicle #1.

An unknown MV struck a fire hydrant located on the sidewalk in front of Whole Foods. Fire department and water department were notified. Four photos were taken of the hydrant by me and given in hand to the It bureau.

Operator of MV1(Ma reg 866MF0) stated he was traveling EB on Washington St when his vehicle was struck in the rear. Operator of MV1 stated he was stopped at the red light at the intersection of Jackson Rd. Operator of MV2(AMB1406) stated he was traveling EB on Washington St when he struck the rear of MV1. Operator of MV2 stated he observed a vehicle coming down from Lewis Terrace and was concerned they were not gonna stop at the intersection. This distraction caused MV2 to strike MV1 at a slow rate of speed. Both vehicles sustained minor damage and did not require a tow. The passenger and driver of MV1 were observed by the medics. The passenger was transported in the ambulance for undisclosed minor injury. The driver refused to sign the medical release. 5 Photos were taken of both vehicles showing the damage. I put the memory card in an envelope and left it for the I.T. Bureau.

On 7/20/19 at 23:24 hrs I responded to the area of 552 Commonwealth Avenue for a report of a MVA. Upon arrival it was determined MV#1 had struck two unoccupied parked vehicles in the eastbound parking shoulder. The operator of MV#1 stated he was on his phone while driving and when he picked his head up another vehicle coming westbound was in his lane so he swerved to avoid collision and struck the parked vehicles. The impact caused MV#1 to spin and came to final rest facing Westbound. All vehicles were towed from he scene by Tody's towing. The operator of MV#1 was transported to the hospital for potential injuries. The Operator of MV#1 is being issued MA Citation T1442692 for 90/24 OUI Drugs, 89/4a Marked Lane Violation and 90/13B Impeded Operation/Phone Use and MA Citation T1442693 for 90/24 Operating to Endanger.

OPMV#1 Was traveling Westbound on Washington St in the left travel lane when he reached the traffic light in traffic at Washington St and Commonwealth Ave. OPMV#1 Attempted to enter the right travel lane colliding with the OPMV#2. OPMV#1 States he signaled to change lanes, looked over his shoulder and when he entered the lane he was struck by the OPMV#2. OPMV#2 Was traveling Westbound on Washington St in the left travel lane when she reached the traffic light at Washington St and Commonwealth Ave. OPMV#2 Attempted to enter the right travel lane and the OPMV#1 entered the right lane at the same time colliding with the OPMV#2. OPMV#2 Stated she signaled to get into the right lane and when she entered the lane to pass the OPMV#1 thats when she was struck by the OPMV#1 entering the right travel lane.

Operator of Motor Vehicle #1 was sitting in his vehicle parked at a metered spot facing Westbound on Beacon Street. Operator of motor vehicle #1 stated that he felt the vehicle move and walked outside and noticed that the rear break light was smashed. Operator of Motor vehicle #1 stated that he stopped a Toyota Camry that he thought struck the vehicle however there was no damage to camry and the operator stated that he did not hit his vehicle. Operator of motor vehicle #1 stated he was unable to locate the vehicle that struck his car. I canvassed the surrounding businesses and all businesses had negative results for cameras outside that would be able to capture the hit and run.

The operator of MV#1 stated he was traveling straight ahead when he was struck in the D/S rear corner. The operator of MV#2 stated that she was struck by MV#1 in the P/ S front.

MV#2 was traveling south on Waverly Avenue when it was struck from Franklin Street by MV#1. -Operator of MV#1 stated she was traveling east on Franklin Street and that she stopped at the intersection. She stated as she was about to enter Waverly Avenue from Franklin Street and after checking to ensure she was clear to enter, she collided into MV#2. She stated he was flying up the road. -Operator of MV#2 stated he was traveling south on Waverly Ave when he was struck by MV#1. Operator of MV#2 stated the collision spun his vehicle 180 degrees. He stated he was facing the direction he was just comming from. He stated he didn't think she (Operator of MV#1) stopped. - I observed the front of MV#1 sustained heavy damage. The hood was bent in, the bumper was cracked and hanging, and fluids were leaking from the vehicle's engine. I next observed the damage to the doors on the right side of MV#2. I observed the base of the doors were pressed in, those doors would not open and the tires were grinding against the flared edges of the wheelwells. - There were no injuries reported at the scene, however, both operators were checked by Ambulance 1. Newton Fire responded to scene to contain the Hazmat spill from MV#1. MV#1 was towed from the scene. OpMV#1 was cited (T2015541) for Failure to come to a Complete stop at stop sign. I transported Operator of MV#1 to Tody's towing from the scene.

Bus#1 was turning right from Chestnut Street to Woodward Street when it struck a traffic signal light knocking it over; the post then landed on the fence of 702 Chestnut street causing minor damage to the fence. -Op Bus#1 stated he was traveling north on Chestnut Street and was making the turn from Chestnut Street on to Woodward Street when he observed a an unknown MV coming toward him as he was turning onto Woodward Street. He stated the operator of the unknown Mv who did not observed the wide angle which the bus needed to make the turn safely and continued forward toward the bus . Op Bus#1 stated he tightened his turn in order to avoid colliding with the oncoming MV striking the post. Op bus#1 stated he pulled over to asses the damage to the bus and the post cause by the collision. He stated the unknown MV continued traveling in an unknown direction. Op bus# stated he did not strike the vehicle. He contacted his the shift supervisor to report the incident. -Next spoke to the resident of 702 Chestnut Street who exchanged information with the MBTA shift supervisor regarding the damage to the fence. - I observed damage to the right rear panel next to the bus's rear door. I observe green paint transference on the panel a result of striking the base of the traffic signal post. I next observe the post was leaning on the fence. I observed bends and dents on the surface of the top bar of the fence and paint chip offs. I took picture of the damage to the bus and the property involved. - There were no injuries reported at the time of the collision and no vehicle was towed from the scene.

Owner of vehicle 1 was informed by Marina Feldman that she witnessed vehicle 2 back into vehicle 1 while it was parked in the lot of 677 Winchester St. Operator of vehicle 2 stated that he was unaware that he hit vehicle 1.

Operator 1 states she was stopped at the red light on Beacon St facing Eastbond at the intersection of Grant St when she was struck from behind by vehicle 2. Operator 2 states that she was traveling Eastbound on Beacon St. She didn't see vehicle 1 until it was to late to stop, and struck vehicle 1 from behind. There were no injuries. Both vehicle sustained moderate damage and vehicle 2 was towed by a private company.

The OP. of MV#1 states while travelling westbound on Washington Street., MV#2 suddenly came out from the side and entered her lane. She tried to accelerate past MV#2 but was still sideswiped. The OP. of MV#2 states she was attempting to leave 1897 Washington St. (Woodland Golf Club) to make a left turn onto Washington St. Traffic was stopped for a nearby red light and a vehicle on Washington St. gave her room to exit. She slowly pulled out into traffic and thought MV#1 was slowing down to let her out as well, that's when there was a collision.

Operator 1 of MV 1 was slowed in traffic on Washington st (wb) by Quinobequin Rd when MV 2 collided with the back of MV1. Moderate damage to MV1, no injuries to Operator 1. Operator 2 of MV 2 was traveling w/b on Washington St by Quinobequin Rd when he stated MV1 stopped abruptly and was unable to stop. MV2 collided with MV1. Moderate damage to MV2, no injuries to Operator 2. No citations issued at this time.

MV#1 was traveling south on Centre Street when it was struck by MV#2. -OpMV#1 states she was stopped at the intersection of Centre Street and Commonwealth Ave traveling south when she was struck by MV#2. -OpMV#2 states he was stopped at the intersection of Centre Street and Commonwealth Ave traveling south when he rear-ended MV#1. OpMV#2 states he was looking left observing traffic to proceed when he struck MV#1. -Neither party reported injuries at the scene and declined medical attention. Neither vehicle was towed from the scene.

Operator of MV1 stated he was waiting at the set of lights to turn left onto Quinebequin road from Washington when MV2 rear ended MV1. Operator of MV2 stated he saw the light turn green to take a left from Washington St to Quinebequin Rd when MV1 stopped due to traffic heading East on Washington St and rear ended MV1. MV1 had minor damage to the rear bumper. MV2 had minor damage to the front bumper. Passenger in MV1 was transported to NWH for kneck pain.

I responded to the intersection of Washington St @Beacon St at 1524 hrs 09/21/2019 for a two car MVA, no injuries reported. I arrived in the area and observed the two involved vehicles stopped curbside along the Eastbound side of Washington St. near Beacon St. Both operators were out of the vehicles, uninjured, standing on the sidewalk upon my arrival. V1; MA Reg 39WS51, 2016 Mazda/CX5 color blue. V1 Operator; Zannis, John MA OLN#S46913063. V2; MA Reg 1RXS11, 2019 GMC/Yukon color black. V2 Operator Pamela Broderick MA OLN#S85148110. The two vehicles were both traveling eastbound on Washington St.(public way) in traffic. As they came approached the intersection of Washington St. @Beacon St., V1 collided into the rear of V2. V2 sustained minor damage to the center rear bumper. V1 sustained moderate front end damage primarily on the left front/drivers side. The drivers airbag deployed at the stirring wheel. The two vehicles were both drive able and did not require towing service. The operators were advised and were given the crash report number.

The operator of vehicle 1 stated he was traveling eastbound on Washington St in the left lane when vehicle 2 crashed into his passenger side as vehicle 2 attempted to make a U turn. The Operator of vehicle 2 stated he was traveling Eastbound on Washington Street with his emergency lights activated the entire time and attempted to merge from the right lane to the left lane of Washington St and vehicle 1 came up along side him causing the vehicles to crash.

Oper # 1 was traveling EB on Washington Street and attempted to switch into right lane but struck veh # 2. Oper # 1 admitted it was her fault. Given MA Citation # T1441516 for unsafe lane change. Veh #1 was towed by Tody's. Oper # 2 stated she was traveling EB on Washington Street when she was struck by vehicle # 2 on her left side. no injuries.

Operator of MV1 stated he was traveling southbound on Centre Street when he observed two people in the crosswalk. MV1 stated he slowed down and came to a stop to let the pedestrians cross the road. As he came to the stop he observed MV2 not slowing down and hit the rear of his vehicle. Operator of MV2 stated he was traveling Southbound on Centre street when he took his eyes off the road for a brief second and rear ended MV1. MV1 had damage to the rear of his vehicle. MV2 had significant damage to the front of his vehicle. MV2 was towed by Todys. Operator of MV1 was able to drive his vehicle.

Vehicle 1 was traveling S/B on winchester st when it collided with parked vehicle 2 in front of 229 Winchester st. Vehicle 2 was pushed into parked vehicle 3. Vehicle 2 and 3 were unoccupied at the time of the accident. Operator 1 had minor injuries and was transported to the NWH by Cataldo ambulance 1. Vehicle 1 and 2 were towed by todys towing. Operator 1 was issued ma Citation T2015491 for c90s24 OUI (Liquor), c90s24 Negligent Operation and c89s4A Marked Lanes violation. See incident #19041837 in regards to this accident for charges.

I responded to the intersection of Commonwealth Ave @ Temple St for a two vehicle accident. The operator of MV1 (MA Reg: 931NK5) stated that she was driving eastbound on Commonwealth Ave when MV2 attempted to cross Commonwealth Ave on Temple St. MV1 stated that MV2 pulled out too quickly and she could not stop in time. I spoke with the operator of MV2 (MA Reg: 9LR479) who stated that she was stopped at the stop sign on Temple St at Commonwealth Ave. MV2 stated that she started to pull out onto Commonwealth Ave to cross over it, and did not see MV1 prior to the accident. MV1 sustained heavy front end damage. MV2 sustained moderate passenger side damage. Both vehicles were towed by Tody's. All parties involved were evaluated by medics and signed a patient refusal.

Operator of MV1 stated he was stopped on Walnut Street eastbound at the red light when MV2 made a left turn from Centre Street onto Walnut Street and struck the front driver side of his vehicle. Operator of MV2 stated she was traveling Northbound on Centre Street, making a left turn on Walnut Street when she struck the left side of MV1. MV1 had damage to the front driver side bumper and headlight. MV2 had damage to the driver side door and rear left passenger door. Both parties stated they were not injured.

MV#1 was parked in front of 283 Tremont St with front driver side damage. The owner of MV#1 came out at approximately 0730 hours and noticed the damage. It is unknown who the owner or operator of MV#2 is.

On October 12, 2019 at approximately 2347 hours, I responded along with multiple NPD units to a report of a MV into a pole at the intersection of Washington St and Commonwealth Ave. Upon arrival, I met with Officer Boyle who directed me to a 2007 Jeep Wrangler(MA Reg:7KK957) that was popped up onto the curb and through a traffic light pole in the intersection on the Westbound side of commonwealth Ave. I was then directed over to the vehicles operator, Sebastian Pike. Sebastian was outside of the vehicle walking around when I had arrived. Sebastian was directed to sit down for now due to airbag deployment from the accident. Sebastian was not complaining of any pain and did not need any medical attention. I then asked Sebastian what happened tonight. Sebastian stated he had just left a friends house and was driving the Jeep Westbound on Commonwealth Ave. He was driving through the intersection with Washington St when he became distracted and looked away from the road. At this point, his Jeep had jumped the curb and crashed into the traffic light pole. The traffic light pole was knocked down and blocked the road Westbound on Commonwealth Ave. Cruisers were used to block the road until Daegle Electric had arrived on scene to clear away the debris. The Jeep was towed by Tody's and a towed motor vehicle form was filed. Due to Sebastian's failure to pay attention, Sebastian Pike was issued Massachusetts Uniform Citation #T2079375 for 90/24 Negligent Operation and 89/4a Marked Lanes Violation. A copy of the report was attached to the citation and submitted to the police prosecutors box as a Criminal Application. Sebastian's Father, Jeffrey Pike, had arrived on scene and drove his son home for the night. All aforementioned streets and Avenue's are public ways in the Commonwealth of Massachusetts.

Operator of vehicle two stated that while tuning from the west bound lane of Watertown St onto the north bound lane of Eddy St vehicle one struck the rear driver's side of his vehicle. Operator of vehicle two stated that he was not injured and his vehicle did not require a tow. Operator of vehicle one stated that while driving east bound on Watertown St she could not stop her vehicle time to prevent it from crashing into vehicle two. Vehicle one had damage to the front passenger side and did not require a tow. Operator of vehicle one stated that she was not injured and her vehicle was towed by AAA. Witness Christina Colantonio stated that she was behind vehicle two and saw vehicle one " speeding and crash into the rear passenger side of vehicle two. Operator of vehicle one was cited for CO 19/75, failure to use care. Watertown St and Eddy St are pubic ways in the city of Newton.

On 10/18/2019, at 1526 hrs, I responded to the intersection of Washington St. and Myrtle St. for a two car MVA. Upon arrival, I observed damage to MV1s front drivers side and damage to MV2s front drivers side. The driver of MV1 stated he was attempting to take a left onto Washington St. into the westbound lane when he collided with MV2 who was stopped in the eastbound lane of Washington St. Both drivers stated that eastbound lane traffic was stopped. The driver of MV2 stated she was stopped in traffic in the eastbound lane of Washington St. when MV1 pulled out from Myrtle St. and collided with her. Cataldo received four patient refusals and all vehicles were able to drive safely from the scene.

Operator 1 stated he was stopped in traffic Southbound on Centre St at Cypress St. While operator 1 was stopped he was struck from behind by vehicle 2. Operator 2 stated he was traveling Southbound on Centre St when the vehicle in front of him stopped suddenly. Due to the rain and slick roadway he was unable to stop in time, striking vehicle 1 from behind. Vehicle 1 sustained minor damage, and vehicle 2 sustained moderate damage and was towed by Tody's. There were no injuries.

Operator of MV1 (Mass. Reg. 9VV485) states he was traveling east bound on Commonwealth Ave when MV2 pulled out into his lane of travel onto Commonwealth Ave from Temple street causing him to collide with MV2. Operator of MV2 states she was traveling south bound on Temple Street when she crossed over the Commonwealth Ave carriage lane and then Commonwealth Ave. She states there were no cars coming on Commonwealth Ave when she entered the roadway and MV1 suddenly appeared and was traveling at atleast 60 mph and collided with her.

Operator of MV1 (Mass. Reg. 4WB261) states he was driving south bound on Hammondswood Road and approached the intersection with Beacon Street. MV1 stopped at the stop sign and turned right onto Beacon Street when he saw 2 police motorcycle units approaching from the distance on Beacon Street. The police motorcycle units did not have their emergency lights or sirens on. MV1 stopped on Beacon Street after he made his turn. MV1 was struck from the rear. Operator of MV2 (Mass. Reg. 4AL429) states he was operating south bound on Hammondswood Road behind MV1 when they approached the intersection with Beacon Street. MV1 stopped and took the right turn onto Beacon Street, MV2 stopped and took the right turn after MV1. MV1 abruptly and unsafely came to a stop on Beacon Street causing MV2 to rear end MV1. Operator of MV1 was determined to be operating MV1 without a drivers license and was subsequently issued a criminal application (T1440757) in hand for unlicensed operation of a motor vehicle (Mass General Law Ch.90 Sec. 10). A licensed operator came to the scene to drive MV1.

Operator of MV1 (Penn. Reg. CKM160) states she was driving east bound on Commonwealth Ave in the left lane when MV2 turned into the right side of her MV. Operator of MV2 (Mass. Reg. 2NB453) states she was driving east bound on Commonwealth Ave in the right lane when she turned left into MV1. Operator of MV2 states she was looking at the cars ahead and did not notice MV1 when she attempted to turn left.

On 11/17/2019, while assigned to N494, I, Officer Conary, responded to Commonwealth Ave at College Road for a MVA. Upon arrival. I spoke to the operator of MV1 who stated that he was traveling Northbound on College Road when he stopped at the stop sign to take a left turn to proceed straight onto Mount Alvernia. Operator of MV1 stated that he thought he had enough time to make the turn but MV2 struck MV1's left side. Operator of MV2 was traveling Eastbound on Commonwealth Ave when MV1 came across the street. MV2 hit MV1 on the side. Operator of MV2 stated that he could not stop in time. Operator of MV2 was a Uber driver and had a passenger in the back. All parties met with medics, all refusals. MV2 was towed by Tody's. MV1 was towed by Operator's personal tow.

MV1, MV2, and MV3 were stopped in traffic on Washington St westbound (public way) when MV4 struck MV3 from behind, causing a chain accident. MV4 sustained heavy front end damage and was towed by Tody's. MV3 sustained both front and rear end damage, and was towed by Tody's. MV2 sustained both front and rear end damage. MV1 sustained rear end damage. MV4 operator stated she was unsure how and why she struck MV3 but believes she fainted and blacked out. No driver said they were injured and all signed medical refusals with the medics.

MV#1 was traveling south on Commonwealth Ave as it was it was merging it collided with MV#2. - OpMV#1 stated he was traveling south on Commonwealth Ave as the road was about to merge from two lanes into one. He was on the left most lane, when he observed a black car come up beside his and tried to get in front of him. OpMv#1 stated he did not know about the vehicle until it collided with his front tire. -OpMV#2 stated he was traveling south on Commonwealth Ave as the road was about to merge from two lanes into one. He was on the right most lane, where he observed a large white landscaping truck merging into him.OpMv#2 felt the collision with the truck's front tire. - I conducted an inspection of both vehicle. I observed no damage to the front tire or the right side of MV#1. I did observe small scratches on the right rear passenger door of MV#2 that resulted from the collision with MV#1. - Both Operators moved off the road in front of 18 Old Colony Rd to safely handle the collision matter. There were no injuries reported at the time of this report. Neither vehicle was towed from the scene and OpM#1 was cited for Operating After Suspension (T2079504). See report19050505 for criminal application. MV#1 was left legally parked in front of 18 Old Colony road waiting to be picked up by another company employee. OpMV#1 took an Uber home from the scene.

Operator of MV1 (MA reg. 6SB554) states she was traveling westbound in the left travel lane on Washington Street when she was struck by MV2 on the passengers side of her car. Operator of MV2 (MA reg 9KJ369) states he was traveling west bound in the right travel lane on Washington Street when he attempted to turn into the left travel lane and accidently struck into MV1.

MV#1 was traveling north on Hammond Street as it crossed the intersection with Commonwealth Ave when it collided with MV#2. -OpMV#1 stated she was traveling North on Hammond Street. She arrived at the intersection, observed the flashing red light, stopped and as she proceeded MV#1 came out of nowhere and she collide with MV#2. -OpMV#2 stated she was traveling west on Commonwealth Ave when MV#1 was struck by MV#2. OpMV#2 stated she crossed the intersection she felt the strike to the MV#1. She stated the collision was a surprise because she did not see where the strike came from until after the collision. -I next interviewed the passengers of MV#2, in Spanish, at the scene and their statements coincided with those made by OpMV#2. They added that they did not see OpMV#1 slowdown at all as it came at them. - Both vehicles were towed from the scene and one party was transported to Newton Wellesley Hospital for further care. No citations were issued at the time of this investigation.

MV1 was heading EASTBOUND on Washington St when the turned onto Cross St from the left lane. MV1 stated that he was distracted by a driver to his right weaving in and out of the lines so when he took his left hand turn, he didn't see MV2 approaching him from the oncoming lane. There were no witnesses to the crash. MV2 was heading WESTBOUND on Washington St in the left lane when it collided with MV1. Both parties signed refusals with the medics. Both vehicles were towed by Tody's.

MV1 was yielding on Quinobequin Rd to the vehicles driving Southwest on Washington St. MV1 was preparing to bear right onto Washington St when she was rear-ended by MV2. MV2 saw MV1 yielding, proceeded to look both ways on Washington St and thought MV1 had gone but they had not. MV2 collided with MV1 at the intersection of Quinobequin Rd and Washington St at 2221 Washington St. MV1 had minor scratches to the rear bumper area and MV2 had heavy front end damage to the front bumper and headlights. Both cars drivable, no injuries.

On 12/9/2019, at 1526 hours, I responded to the intersection of Washington St and Lewis Ter for a two car MVA. Upon arrival, I observed damage to both vehicles front ends. No injuries were reported. The operator of MV1 stated she was on Washington St (WB) attempting to take a left onto Lewis Ter when she collided with MV2 who was traveling EB on Washington St. The operator of MV2 stated he was traveling eastbound on Washington St when MV1 turned in front of him and they collided. It appears that MV2 had the right of way. MV1 was towed from the scene.

At approximately 2250HRs on Friday, December 13, 2019, I was dispatched to the area of 275 Washington St. for a report of a hit and run crash; Newton Officers and a State Police Officer canvassed the area with negative results. Upon speaking with the operator of MV1 (MA Pass:1CTB36), he stated that he was traveling Westbound on Washington St., when he observed a black Honda mini van or SUV, MV2, approach him in his passenger's side mirror and then attempt to enter his lane of travel, striking the rear of MV1. Upon speaking with the two witnesses who were traveling behind MV1 and MV2, they concurred with the operator of MV1's account. The witnesses described MV2 as a black Honda or Toyota mini van or SUV. I observed damage to the right rear quarter panel, trunk area and right rear tire of MV1. Todys Services arrived and took custody of MV1. I filled an NPD Towed Motor Vehicle Form. **Update as of 1800HRs on 12/15/2019** I was made aware that the operator of MV2 in this incident, now identified as MA Pass: 8HD155, submitted a Commonwealth of Massachusetts Motor Vehicle Crash Operator Report on 12/14/2019. In said report, the operator admits to striking MV1 and stated that he pulled over on Peabody St., where he remained for approximately 15 minutes awaiting MV1. When MV1 did not arrive, the operator of MV2 attempted to drive away, but realized the damage was too severe and returned to the same spot on Peabody St. I viewed MV2 at Clay Auto Body and observed damage to its left fender/wheel well area, as well as a deployed passenger's side airbag. Upon speaking with the operator of MV2, he reiterated the same information he stated in the above summary of his Commonwealth of Massachusetts Motor Vehicle Crash Operator Report. In the presence of his parents, I advised him on the proper steps to take when involved in an accident, such as the proper exchange of information and the potential ramifications of not doing so. Due to the operator of MV2 being 17 years old, being issued his driver's license 9 months ago and his evident remorse/confusion surrounding the incident, I will not be seeking any charges. I updated the operator of MV1 and provided both parties with an NPD Crash Data Exchange Form.

Operator of vehicle 1 states she was traveling westbound on Washington St when vehicle 2 turned quickly in front of her giving her no time to stop and they crashed. As a result of the crash her car rolled backwards into the opposite curb. Operator of vehicle 2 states he was traveling eastbound on Washington St and was waiting to turn left onto Concord St. The traffic light on Washington turned yellow and he went to turn when vehicle 1 came at him at a high rate of speed and hit him head on causing his vehicle to spin around and strike vehicle 3 on Concord St head on. Operator of vehicle 3 stated he was stopped on Concord St at the light to Washington St when vehicle 1 struck vehicle 2 and pushed him into his vehicle. After the accident I spoke to a witness who stated he had video of the incident. Jose Buenrrostro works at 2310 Washington St and was able to show me the footage. In the footage you can see vehicle 1 not slow down for the yellow light on Washington which turns red before she reaches the intersection. She then attempts to swerve around vehicle 2 but was unable to causing the collision. All three vehicles were towed by todys. The operator of vehicle 1 was transported to Brighams Hospital by Cataldo for injuries. Both other operators signed patient refusals with the medics.

Operator of vehicle 1 stated he was in the right lane of the eastbound side of Washington St with his right blinker on waiting to turn right into 2000 Washington St. As he proceeded to turn right vehicle 2 came from his left around the front of his car clipping the front end. Operator of vehicle 2 stated he saw vehicle 1 stopped ahead of him but did not notice his turn signal on and thought he was just stopped so he attempted to go around him from the left lane and collided with him. Neither operator reported any injuries and neither vehicle required a tow.

At approximately 0500HRs on Tuesday, December 17, 2019, I was dispatched to a 2 vehicle MVA in front of 38 Commonwealth Ave. Upon arrival, I spoke with the operator of MV1 (MA CON: S25585) who stated that he attempted to back up his vehicle (Westbound) and accidently struck MV2 because he was not able to see it in his mirror. Upon speaking to the operator of MV2 (MA CON: S13628), he stated he was traveling Eastbound as MV1 was backing up. He then stopped MV2 and when MV1 did not, he activated his horn which had no effect and MV1 backed into him. I observed MV1 to have no discernable damage. I observed MV2 to have sustained damage to its front bumper, grille, hood, passenger's side fender and both head lamps. MV1 had a single passenger who reports he did not witness event in question, as he was focused on eating at the time. No one involved reports any injuries. I cleared without incident.

Operator of vehicle #1 stated he was travelling Westbound on Washington St. The road conditions at that time was covered in Slush and ice. Operator #1 stated as he was rounding a slight bend in the Rd. His vehicle continued to slide to the left and into oncoming Eastbound traffic. Vehicle #1 Then collided with vehicle #2. Operator of vehicle #2 stated he was traveling Eastbound on Washington St. when vehicle #1 slid into his travel lane, striking his vehicle head on. He stated vehicle #2 then Struck vehicle #3 which had been driving eastbound to his right. Operator of Vehicle #3 stated he was travelling Eastbound when his vehicle was struck by Vehicle #2 which caused his vehicle to spin out of control and into the Westbound travel lane where his vehicle struck Vehicle #4. Vehicle #3 then struck the fence owned by the Woodland Golf Club. Operator #4 stated she was travelling Westbound when vehicle #3 spun out of control and struck her vehicle. Operator stated she attempted to stop but was unable to due to the poor Rd. conditions.

Mv#1 was traveling north on Centre Street when it was involved in an argument with Mv#2, resulting in the motor vehicles colliding(ref inc 19055516). -OpMv#1 stated that after the confrontation Mv#2 cut him off striking his vehicle. -OpMv#2 stated that after the confrontation Mv#1 was trying cut him off striking his vehicle. - Mv#2 Pass1 and Mv#2 Pass2 statements coincide with those made by OpMv#2. -Wit#1 he observed Mv#1 and Mv#2the at the intersection of Commonwealth Street and Centre Street. Mv#1 on the right lane and Mv#2 on the left turning lane. He stated the parties exchanging words at the intersection and a few minutes both parties spat on each other's vehicles. When the traffic control light changed he observed Mv#2 speed off and Mv#1 suddenly changed lanes after it. They stated that they observed that when both vehicle were on Commonwealth Avenue Mv#2 cut off Mv#1, causing the vehicles collided. Mv#1 threw a cup of coffee at Mv#2. Wit#1 and Wit#2 stated they intervened and de-escalated the situation until police arrived. - None of the parties reported injuries to them at the scene not either vehicle was towed. Op MV#1 was cited for Reckless Operation Ch 90s24. (Cit#2079508) by suddenly changing lanes and cutting across the intersection to follow Mv#2. Op MV#2 (Cit#2079507) was cited because his driving caused the collision by shifting lanes with out signaling, cutting off MV#1.

On 12/23/19, at 1919 hours, I responded to the area of 807 Washington St. for pedestrian struck by a motor vehicle. Upon arrival with Cataldo Medics and Newton Fire, I observed Catherine Chapman being treated in the westbound lane a few feet in front of a 2010 Hyundai Elantra (MA Reg. 4KH 743). The vehicle had a shattered windshield. Ms. Chapman appeared to be wearing a dark colored jacket and dark pants. The operator of the motor vehicle, Emily Shuman, stated she was traveling westbound in the outside lane when Ms. Chapman stepped in front of her car from the eastbound lane. I also spoke with a witness, Malcolm Robinson, who stated Ms. Chapman was crossing the street from the eastbound lanes with two other people. The other two people stopped in the middle of the street but Ms. Chapman kept walking and was struck. The other two people were Victor Polk and Halimah Polk, both of which confirmed the witness statement. Ms. Chapman was transported by Cataldo to Brigham & Womens Hospital with non-life threatening injuries. Todys towed the vehicle and pictures of the scene were submitted to IT.

Operator 1 states he was stopped on Beacon St Westbound waiting to make a left onto Glen Ave. While stopped, vehicle 1 was struck from behind by vehicle 2 and pushed through the intersection into a City of Newton fire hydrant, and then into a utility pole. Operator 2 stated that she was traveling Westbound on Beacon St, she didn't see vehicle 1 stopped, and stuck it from behind. Vehicle 1 sustained major damage and was towed from the scene by Tody's. Vehicle 2 sustained minor damage. The passenger of vehicle 1 sustained a laceration to her lip; however, refused medical attention.

On 01/02/2020, at 1813 hours, I responded to the area of Washington St and Centre St for a two car MVA. Upon arrival, I observed MV1 to have minor damage to the front drivers side wheel well. MV2 had minor damage to the passenger side.The operator of MV1 stated he merged onto Washington St from Centre St. While attempting to change lanes he did not see anyone and collided with MV2. The operator of MV2 stated he was traveling westbound on Washington St when MV1 changed lanes into his and collided with his passenger side. No injuries were reported and both vehicles were driven from the scene.

On 1/5/20, I responded to the area of Commonwealth Ave @ Chestnut St for a 2 vehicle accident. On arrival, I spoke with the operator of MV1 (MA Reg: 95VC76) who stated that she was travelling eastbound on Commonwealth Ave. MV1 stated that she had a flashing green light while she proceeded through the intersection. As MV1 was travelling across Chestnut St she was struck on the passengers side by MV2. After the collision, MV1 was directed into a light pole and struck the light pole at Commonwealth and Chestnut St. (Pictures turned over to the IT bureau). The operator of MV2 (MA Reg: 95VC76) stated that he was travelling northbound on Chestnut St approaching the intersection of Commonwealth Ave. MV2 stated that he had a flashing traffic signal, and proceeded into the intersection. I asked MV2 if it was flashing yellow or red, and the operator MV2 stated he was unsure. As MV2 proceeded into the intersection, he collided with MV1. The operator of MV1 was transported to NWH for further evaluation. The occupants of MV2 were evaluated by the medics and signed refusals. Both vehicles were towed by Tody's Towing.

The operator of MV#1 stated he was traveling westbound on Franklin Street and came to a stop at the stop sign. He stated he proceeded to cross the intersection at Waverley ave when he struck MV#2. The operator of MV#2 was traveling southbound on Waverly ave when she was struck in the drivers side by MV#1. MV#2 had airbag deployment and the operator was transported by the medics to NWH. There was a construction detour just being set up at the time of the accident. It is possible that the view was obstructed for the operator of MV#1.

MV1 stated she stopped at the stop sign on Eddy St then attempted to turn left on to Watertown St when MV2 "hit me." MV1 sustained heavy passenger side damage and was removed from the scene by Tody's Towing. MV2 stated he was travelling w/b down Watertown St when MV1 "cut in front of me and I couldn't stop in time" subsequently striking MV1 causing minor damage. After contact with MV1, MV2 spun and struck MV3 who was travelling e/b on Watertown St causing front end damage. MV2 was removed from the scene by Export Towing & MV3 was removed by Tody's Towing. All parties were evaluated by Cataldo Ambulance who cleared with patient refusals.

The Operator of vehicle 1 stated he was stopped on Washington Street at Grove Street to take a left turn onto Grove Street. While he was sitting there, waiting for cars to pass going the other direction he was ht by vehicle 2 in the rear passenger side. The Operator of vehicle 2 stated he was traveling East on Washington Street when he crashed into vehicle 1 which was stopped in the middle of the road. He stated by the time he saw the vehicle it was too late to avoid a collision. The crash caused minor damage to the bumper of vehicle 1 and major front end damage to vehicle 2.
