On 01/04/19 I responded outside of #54 Needham St for a two vehicle crash. Veh #1 was a delivery box van who was driving due North on Needham St. The operator of veh #1 stated he was attempting to take a left into #55 Needham St. when veh #2 came from behind him on his left and he struck veh #2 as he turned. The operator of veh # 1 was taken by Ambulance #1 to Newton-Wellesley Hospital for an evaluation after complaining of pain in his chest area. The passenger of veh #1 stated everything happened "so fast" he didn't see what transpired. Moderate damage to veh #1. The operator of veh #2 stated he was traveling due North on Needham St. and was behind veh #1 for a long stretch of the road. The operator of veh # 2 stated veh #1 put his right directional on three different times but didn't turn. He stated veh #1 had his right directional on and was in the right lane when he went to pass him. Veh #1 then turned into his rear passenger side as he passed. Moderate damage to veh # 2. The operator of veh #2 stated the operator of veh #1 was walking around and yelling about the accident for a period of time and then eventually grabbed his chest and fell to the ground complaining of pain. From the result of my investigation it appears as if veh # 1 attempted to turn from the wrong lane and veh # 2 was passing in the "suicide lane", also wrong lane. There will be no citations issued at this time as both operators were given verbal warnings.

OPERATOR OF MV 1 WAS TRAVELING WEST ON CALIFORNIA STREET WHEN SHE HIT HER BRAKES. DUE TO THE VERY ICY CONDITIONS OF THE ROADWAY MV 2 WAS UNABLE TO BRAKE IN TIME AND STRUCK MV 1 IN THE REAR. On 2/9/19 I responded to 54 Ash St and met with Ms. Zardar Hovhannisyan in regards to a past hit & run to her vehicle. Hovhannisyan parked MA reg 1EC474 on Ash St (a northbound public one way street) just prior to Commonwealth Ave around 0800 hrs this morning. When she returned around 1000 hrs, she noticed moderate damage to the front left area of the vehicle. There was minor debris on the ground consistent with the damage I viewed. It should be noted that there are parking stalls parallel to where the vehicle was parked, with several residents of Norumbega Gardens coming and going quite frequently. It is not believed that there are cameras facing the specific area where the collision occurred. Hovhannisyan was advised to contact her insurance company.

Vehicle 1 was parked in a parking space perpendicular to vehicle 2, in front of Splash. The operator of vehicle 1 went inside of Starbucks from 1300-1445 hours. When she arrived back to her car she noticed damage to the drivers side of the vehicle. I observed there to be moderate damage to this side of the vehicle. It appeared that vehicle 2 backed into the drivers side of vehicle 1. The operator of vehicle 2 left from the scene and failed to provide their information. There are no witnesses or suspects at this time. There are surveillance cameras in front of Splash. I asked the employees in Splash if I could access the camera's, and they explained that they would have to wait until tomorrow to speak with the manager. No one was injured, and vehicle 1 did not need to be towed. On Monday, April 29th 2019, at approximately 1600 hours, I left a voicemail for the owner of Splash, Jamie Mcmanus, in regards to any possible surveillance footage. I requested that he call Newton Police headquarters and ask for me, when he gets the message.

Vehicle #1 was parked on the right side of the road and was unoccupied at the time of the crash. Vehicle #2 was attempting to parallel park behind Vehicle #1 and struck Vehicle #1 in the left rear with the right rear of Vehicle #2. A witness stated they watched Vehicle #2 hit Vehicle #1 in the attempt to park and then drove off after the accident leaving no information. The Witness left a note with Vehicle #2's information along with his information in regards to the accident. Vehicle #1 had moderate damage to the left rear and left side. Vehicle #2's damage is unknown at this time. I attempted to contact the owner of Vehicle #2 via the Boston Police with negative results. I was able to contact a relative of Vehicle #2 who refused to give me the drivers information but stated he would pass the message along to the driver. Ms.CORIOLAN, Roodline contacted me back and stated she was involved in an minor accident while attempting to park but stated there was no to little damage done. Ms. CORIOLAN was advised of the accident report that was filed. I contacted the owner of Vehicle#1 and advised him that I obtained all of Vehicles #2's information and informed him that it would all be in the accident report.

On Wednesday 02/19/2020, at approximately 1500 hrs, operator of MV1 stated she was traveling eastbound on Newtonville Ave, right before Walnut Street, a dark colored pick up truck was traveling behind her. Operator of MV1 stated she was driving below speed limit due to the approaching intersection at Walnut street when MV2 went into the eastbound traveling lane attempting to go around her. While attempting to go around MV1, MV2 side swiped MV1 causing the driver side mirror to break off causing moderate damage to the driver side of her vehicle. Operator of MV1 stated the pick up truck then stopped in front of her and the operator, an adult male began to yell at her then drive off. Operator of MV1 stated she got back in her car and did not get any information and did not have her cell phone to call the police. Operator of MV1 stated she was able to drive her vehicle home that day. No further description of the pick up truck was provided.

Mv#1 owner stated he parked his Mv on Lakewood Rd (opposite #23 driveway) at about 1000hrs this date. Owner stated he returned to his unoccupied parked Mv at about 1045hrs to find fresh light moderate driver's side front end damage with light scrape marks. A witness from #32 left a note indicating a beige or tan Range Rover unknown plates (Mv#2) backed into that front left bumper. I raised the witness who further stated she was on her porch and heard a noise but didn't see a collision. She stated it (#2) backed out from the driveway and observed it perpendicular to #1 trying to maneuver away from it and drive away. I raised occupant of #23 and spoke to #2 operator who was not on scene by cell phone. #2 operator was unaware of any collision. #2 texted pictures of his Mv's rear bumper. One picture appeared to show a light colored scrape on driver's side upper rear end. Mv#1 is dark blue. Mv#2 is bronze. Both parties agreed to exchange information. Both parties were advised the matter would be documented and to proceed further with their respective insurance companies.

Mv#1 (tractor trailer) operator stated he had a clear access road in the parking lot as he was backing to make a delivery into the Whole Foods loading dock. #1 further stated before he backed up, he observed Mv#2 off to an adjacent side corner of the parking lot, which left the access road clear leading to the loading dock. #1 stated at that time while he was backing, #2 attempted to pass his trailer on the right. A collision occurred with the passenger side rear trailer and the driver's side front of #2. #2 operator stated he was attempting to exit the parking lot by the access road leading out to Beacon St. #2 further stated he observed the trailer backing up, at which time he stated he came to a complete stop and was honking his horn to alert #1. #2 stated at that time, #1 did not stop and backed into his driver's side front end. #2 sustained moderate damage. #1 sustained no damage to the rear trailer. Observations of Mv positioning on arrival was:the trailer was almost fully in the center of the access road leaving only about a 3 foot opening from the curb on its right. Mv#2 was in the far right lane of the access road. Although #2 reported he came to a complete stop prior to the colllision, it appeared #2 intended to pass and approached too closely to the rear of the trailer to leave enough safe distance between both Mvs.

Mv#1 was parked unoccupied opposite #8 Burrage Rd (N/B). #1 was sideswiped by an unknown Mv that left white paint transfer damage on #1's driver's side front end side and also struck the driver's side mirror all causing moderate damage. I spoke to the resident at 8 Burrage Rd. The resident stated a visitor in his house, Jonathan Kaplan, witnessed the accident. The resident stated Kaplan told him he observed a white sedan bearing MA Reg CIAF31 (Mv#2) attempting to parallel park between #1 and another Mv. During the attempt, #2 was observed sideswiping #1. Kaplan further stated a male operator got out of #2, left a note on #1 and drove away. Mv#1 owner/operator Ms Joan Beer stated no note was ever found. Later from HQ, I called the witness phone number provided (617-455-2557) by the resident of 8 Burrage Rd. I called that number, received VM and left a message. A query of #2 registration comes back on a '18 white Lexus G350 to a Mr Barry Benjamin of 79 Florence St #510S Newton. I received information Mr Benjamin's phone number was 339-223-0079. This number when called was not Mr Benjamin's. Later from HQ, I spoke to Ms Beer and provided her with the witness plate number observed as the paint color is a match to the damage sustained on her Mv. She was advised insurance company will investigate further based on current information received. Later, the witness Mr Kaplan returned my phone call and provided the following statement: Kaplan stated he was in his driveway outside directly across the street from where the accident was taking place. Kaplan stated the accident was very obvious as damage was unfolding while Mr Benjamin was attempting to squeeze between Mv#1 and another vehicle. Kaplan stated after realizing he (Mr Benjamin) could not fit he started to drive away. Kaplan stated it drew the attention of another witness (unknown) standing on the street when both walked up to Mr Benjamin's vehicle and asked him if he was going to leave a note for the damaged vehicles. Benjamin refused. Kaplan described Benjamin's overall demeanor as "rude." Kaplan stated he told Benjamin he has cameras capturing the incident. Not until then, Benjamin got out of his car and left a note on only the other vehicle reported to be minor license plate damage. Kaplan stated Benjamin never left a note on Mv#1, the vehicle with obvious moderate damage. Kaplan stated he actually viewed the note left on the other vehicle and said it was illegible. Kaplan told Benjamin this and Benjamin yelled to him "mind your own business" and drove away. Kaplan stated his ring doorbell camera was not positioned to capture any of the incident. Traffic Bureau update (Officer Gaudet): On September 8, 2021 I was able to make contact with the operator of MV2, Mr. Barry Benjamin (S42076881). Mr. Benjamin stated he was operating his 2018 Lexus G350 (MA: CIAF31) at the time of the crash. Mr. Benjamin stated he was backing his vehicle into a parking spot and the space ended up being too small. Mr. Benjamin stated the rear of his vehicle crashed into the front bumper area of MV1, knocking off it's license plate. Mr. Benjamin stated he got out of his vehicle and put the plate back on MV1. Mr. Benjamin stated he left a note in the vehicle with his contact information and left the area. I asked Mr. Benjamin if he was aware of making contact with another vehicle in the area. Mr. Benjamin stated he did not make contact with any other vehicle. Mr. Benjamin stated there was no damage to his vehicle and stated he would e-mail me pictures of his vehicle and pictures of MV1 that he took on scene. Those pictures were forwarded to the IT Bureau to be added to this report. Mr. Benjamin also stated in his e-mail, "My wife informed me that the homeowner who contacted the police called my wifes attention to a car up the street. The car was on the opposite side near a dirt road that had been side swiped earlier. My wife said, "That has nothing to do with us" and the homeowner agreed that that situation did not involve our car." At this time, no charges will be filed against Mr. Benjamin for leaving the scene of property damage.

Driver #1 stated she was travelling westbound on Needham St and had entered the middle lane (turning lane ) to turn left into Farm Grill, 40 Needham St when she was hit on the right side by Driver #2. Driver #2 stated she was travelling westbound with her blinker on, moving into the middle turning lane from the northbound lane when Driver #1 came up abruptly beside her and she had no chance to stop. Minor damage to the passenger side of MV#1. MV#1 was driven from the scene. MV#2 has moderate damage to the front drivers side and was leaking coolant. The car was parked in a private parking spot (with permission from Farm Grille ) until a tow could arrive on scene. All parties declined medical attention.

OPERATOR OF MV 2 STATED THAT SHE WAS ON RIVER ST WHEN MV 1 WAS COMING OFF OF AUBURNDALE AVE ONTO RIVER ST AND TRIED GOING AROUND THE TRASH TRUCK AROUND THE SAME MOMENT. MV 2 STATED THAT SHE HAD THE RIGHT OF WAY. BOTH MV'S HAD MODERATE DAMAGE AND NEITHER VEHICLE REQUIRED A TOW. THE FRONT PASSENGER OF MV 2 WAS TRANSPORTED TO NWH BY MEDICS FOR FURTHER EVALUATION.

MV 1 WHO INITIALLY STOPPED ON WASHINGTON ST IN ORDER TO LET A MV TURN LEFT OUT OF THE DRIVEWAY OF THE WARREN HOUSE, LEFT THE SCENE PRIOR TO EXCHANGING INFORMATION WITH THE OTHER INDIVIDUALS INVOLVED. A WITNESS IDENTIFIED AS KATE VELASQUEZ HELLER WAS ABLE TO TAKE A PICTURE OF MV1 WHICH INCLUDED THEIR LICENSE PLATE (MA REG 6NH642) AND IT SHOULD BE NOTED THEIR REAR WINDSHIELD WAS BLOWN OUT DUE TO THE IMPACT. THIS PICTURE, WHICH SHE TOOK ON HER CELL PHONE WAS EMAILED TO ME AND ATTACHED BY THE IT BUREAU TO THIS REPORT. THE VEHICLE BELONGS TO A FEMALE PARTY OUT OF BRIGHTON MA AND THE OWNER DOES NOT APPEAR TO HAVE A MA LICENSE. I HAD DISPATCH CALL BPD TO SEE IF THEY COULD GO BY THE ADDRESS TO SEE IF THE INVOLVED VEHICLE WAS THERE AND TO SEE IF THEY COULD RAISE ANYBODY AT THAT ADDRESS AND I NEVER GOT A RESPONSE REGARDING THIS ACCIDENT. IT IS UNCLEAR FROM THE PICTURE AND VIDEOS I VIEWED WHO IS OPERATING MV1. ALL THREE VEHICLES SUSTAINED MODERATE TO HEAVY DAMAGE. MV2 WAS TOWED BY AAA AND MV 3 WAS TOWED BY TODYS TO THEIR LOT. THE OPERATOR OF MV2 WAS TRANSPORTED TO NWH BY MEDICS TO BE FURTHER EVALUATED AND NO OTHER INJURIES WERE REPORTED. THE OPERATOR OF MV2 ALSO HAS A DASH CAM WITH 2 VIDEOS AND STATED THAT SHE WOULD EMAIL ME THE VIDEOS WHEN THEY HAVE A CHANCE. I VIEWED BOTH DASH CAM VIDEOS WHICH WERE EMAILED TO ME, AND IT CONFIRMS THAT MV 3 CAUSED THE ACCIDENT BY NOT STOPPING AND REAR ENDING MV 2. THESE VIDEOS WERE ALSO ATTACHED TO THIS REPORT BY THE IT BUREAU. UPDATE: ON 11/12/21 I RECEIVED A CALL FROM SALINA LOPES AND HER FRIEND WHO HELPED TRANSLATE FOR HER DUE TO A LANGUAGE BARRIER. SALINA CONFIRMED THAT SHE WAS THE PERSON DRIVING HER VEHICLE AT THE TIME OF THE CRASH AND SHE DIDN'T STAY ON SCENE BECAUSE SHE WAS NERVOUS. SHE WAS ADVISED SHE WOULD BE ISSUED A CITATION VIA MAIL FOR CH. 90 S.10 UNLICENSED OPERATION.

Vehicle #1 was traveling northbound on Centre St. at approximately 25 MPH when they witnessed someone enter the roadway on foot from between two parked cars and attempted to cross the road. Vehicle #1 stated they attempted to stop but was unable to do so and struck the person with the front left corner and windshield of the vehicle. The pedestrian and owner of Vehicle #2 stated he parked his vehicle on the right side of the road and exited the car. He stated he was walking towards the crosswalk and attempted to cross the street from the front of his vehicle. All he remembers is taking a few steps out into the road, being hit, and flying before hitting the ground. He was unaware where he was struck from. Vehicle #1 had moderate damage to the front and windshield and was towed by Tody's towing. Vehicle #2 was uninvolved in the crash and was parked at the time. The owner of Vehicle #2 and pedestrian struck was injured and transported by Medics to Brigham and Womens Hospital for injuries to his head, left arm, and left leg. Operator of Vehicle #1 stated he was uninjured and refused any medical treatment. It should be noted that the accident happened outside the crosswalk and the street lights above where the accident occurred were not working at the time of the crash making the roadway dark and visibility difficult. Photos of the accident were taken and saved on a memory card.

Operator of MV#1 parked her vehicle in a parking spot behind 1188 Centre St at 0800. When she returned to her vehicle at 1200 there was moderate damage to the left rear door of the vehicle. It appears the vehicle was sideswiped by a vehicle that left the scene. White paint transfer indicates the other vehicle would be white in color. At the time of this report, I am waiting to hear back from Keith Scott - Management, regarding cameras. On 5/19/22 I spoke with Jean Lew who stated the only camera that may have potentially faced the parking lot would be the ATM camera. Citizens Bank would only release that information to a police officer. I spoke with Det Koerber about obtaining contact information for Citizens Bank. To be followed up.
