On 01/04/19 I responded outside of #54 Needham St for a two vehicle crash. Veh #1 was a delivery box van who was driving due North on Needham St. The operator of veh #1 stated he was attempting to take a left into #55 Needham St. when veh #2 came from behind him on his left and he struck veh #2 as he turned. The operator of veh # 1 was taken by Ambulance #1 to Newton-Wellesley Hospital for an evaluation after complaining of pain in his chest area. The passenger of veh #1 stated everything happened "so fast" he didn't see what transpired. Moderate damage to veh #1. The operator of veh #2 stated he was traveling due North on Needham St. and was behind veh #1 for a long stretch of the road. The operator of veh # 2 stated veh #1 put his right directional on three different times but didn't turn. He stated veh #1 had his right directional on and was in the right lane when he went to pass him. Veh #1 then turned into his rear passenger side as he passed. Moderate damage to veh # 2. The operator of veh #2 stated the operator of veh #1 was walking around and yelling about the accident for a period of time and then eventually grabbed his chest and fell to the ground complaining of pain. From the result of my investigation it appears as if veh # 1 attempted to turn from the wrong lane and veh # 2 was passing in the "suicide lane", also wrong lane. There will be no citations issued at this time as both operators were given verbal warnings.

Motor vehicle 1 (MV1) was traveling eastbound on Beacon Street, when unknown motor vehicle 2 (MV2 ) pulled out from a parking space in front of 706 Beacon Street, without signaling, and crashed into the front passengers side of MV1. MV1 pulled over to the side of the road to exchange information as MV2 fled the scene eastbound on Beacon Street. The operator of MV1 stated that MV2 appeared to be an older model sedan, possible maroon or red in color, and was operated by a white female in her 60's. MV1 sustained significant front end and passengers side damage. MV2 likely sustained significant front drivers side damage. I was unable to check the surrounding businesses for possible security footage of the crash as all of the surrounding stores were closed when I arrived on scene.

On the above date and time I responded to 25 Curve Street for a report of a motor vehicle crash with property damage " house". Upon arrival I met with the operator Ms. Brown who had an active learners permit and her father Mr. Brown who had an active license who was sitting in the right front of the vehicle.. She stated while pulling into the Church parking lot she misjudged the width of the driveway and doing so had minor contact with the right side of the house belonging to 25 Curve Street. I then spoke to her father who stated the same thing. We went over to 25 Curve Street and met with Mr. Turner the home owner. The process was explained to everyone and everyone had each others information. There were no reported injuries.

On 1/18/19 at approximately 1200hrs, a party who wished to remain anonymous reported observing a tractor trailer travelling on Washington St W/B at #1479 where the street curves to the right struck a traffic light pole in front of Burke & Blackington Funeral Home. The side of the trailer struck the pole. It was reported the operator, a non descript black male, stopped after the collision, picked up the downed pole and moved it onto the sidewalk. Markings observed on the trailer were "Powered Transportation" out of Laurel, Mississippi. Unknown plates. Operator left scene and as of this time, it hasn't been reported by the operator. Photos were taken of the downed pole and submitted to the IT bureau. Daigle Electric notified to respond. Google search of "Powered Transportation" was negative results.

On the above date and time I responded to the area of St.James St at Washington Street for a report of a past hit/run with City traffic signal knocked over. Upon arrival I observed the the traffic signal on St. James Street westbound just before the Washington Street bridge on the roadway. I did observe what appeared to be a sidewalk plow tire impressions on the sidewalk to the traffic signal. I cnanvassed the area with negative results and I am waiting to hear back from Public works who may have been assigned the area to plow the side walks. I also checked for any cameras in the area with negative results. I took pictures of the crash site area and I will have them downloaded at th I.T. Bureau.

On the above date/time I responded to 647 Washington Street for a report of a two vehicle crash. Upon arrival I met with the operator of vehicle two Mr. Weiner, he stated he was stopped in the Whole foods parking lot when vehicle one backed up into his vehicle. They exchanged information but the operator of vehicle one Ms. Adetola left prior to my arrival. I obtained her license information through Mr. Weiner and when I ran the reg plate on vehicle one it came back to Watertown Ford. I advised Mr. Weiner of the process and I will contact Watertown Ford for more contact information on Ms. Adetola to obtain a statement. Update:Jan. 21, 2019 On Jan. 21, 2019 I contacted Watertown Ford advised them of the crash and obtained a phone number for the operator of vehicle one. I spoke to her on the phone she stated the parking lot roadway was covered in snow/ice, she was backing out of the parking spot and vehicle two continued traveling and struck her vehicle. I advised her of the process.

The operator of MV#1 stated that she was backing out of a parking spot in the Star Market Parking lot, #2040 Commonwealth Ave when she was involved in a MV crash. Operator #1 stated that she and the other operator both exited their MV's and even though there was obvious damage to MV#1, the operator of MV#2 refused to provide any information, got back into his MV and left the area. Operator #1 describes MV#2 as a large yellow backhoe that did not display a MV plate and she didn't observe a company name anywhere on the MV. The operator id described as W/M, approx. 40 YOA with brown hair. There is a backhoe type MV presently in the Star Market parking lot and operator #1 stated that is not the other MV involved. I also spoke with a Star Market manager who will view their exterior video footage of the parking area and notify me if it provides any information.

On 02/08/2019, I, Officer Conary, responded to 309 Tremont Street for a past hit and run involving a pedestrian. Upon arrival, I was met by FEBLOWITZ, Colleen, who stated at approximately 1745 hours, she was crossing in the crosswalk at Tremont at Hibbard when her foot was hit by a car. FEBLOWITZ explained to me that she was in the crosswalk and she saw a silver sedan, MV1, traveling Eastbound on Tremont Street approaching her but it was far away where she felt safe to walk across the street. She said she also raised her hand to draw attention to herself. FEBLOWITZ was halfway in the crosswalk when she felt severe pain in her right foot. She looked behind her and saw MV1 was very close to her. MV1 continued to drive down Tremont Street and not stop. There were no witnesses. FEBLOWITZ went to Partner's Urgent Care to be treated for minor injuries. The crosswalk is in a lighted roadway. FEBLOWITZ was unable to gain any other information on MV1. No further incident to report.

On 2/9/19 I responded to 54 Ash St and met with Ms. Zardar Hovhannisyan in regards to a past hit & run to her vehicle. Hovhannisyan parked MA reg 1EC474 on Ash St (a northbound public one way street) just prior to Commonwealth Ave around 0800 hrs this morning. When she returned around 1000 hrs, she noticed moderate damage to the front left area of the vehicle. There was minor debris on the ground consistent with the damage I viewed. It should be noted that there are parking stalls parallel to where the vehicle was parked, with several residents of Norumbega Gardens coming and going quite frequently. It is not believed that there are cameras facing the specific area where the collision occurred. Hovhannisyan was advised to contact her insurance company.

Operator of MV#1 stated he was travelling westbound on Commonwealth Ave with his dog in the front seat when he slid on ice and off the roadway. The roadway was wet from previous snow fall but was very well plowed and sanded. MV#1 hit a curb, drove through a snowbank, hit a tree with the left front of his vehicle and then hit a pole with the right front side of his vehicle. The car then came to a rest in the median. The operator appeared confused by the accident and did not realize the extent of the damage to his car.He asked what he had hit. He continuously asked if it was driveable even though the entire front end was totaled. He was able to provide me with his license and all pertinent information but spoke in a slow manner. There was no odor of alcohol. Operator declined medical attention. When asked if he was on any medication he stated he was on heart medication. We were able to contact the operator's son who was transported to the scene and stated the operator had previously suffered a stroke. David Donahue arrived on scene towed the vehicle to Tody's. A licensed driver picked up the operator. A search of the immediate area yielded negative results for ice in the roadway. A request for Immediate Threat License Suspension/Revocation was filed.

On 03/12/2019, while assigned to WN, I, Officer Conary, met with DRAGAT, who wanted to report a motor vehicle accident have occurred on 03/04/2019. MV1 was parked behind 25 Oak Street in a parking stall. MV2 with a plow attached hit MV1 right rear bumper while plowing the snow. Witness, RAYMOND, asked the plow driver if they wanted MV1 moved before the accident and they said no. RAYMOND saw MV2 hit MV1. DRAGAT did not get the information from the operator or passenger of MV2. Operator of MV2 told her to contact the owner of the MV2, who was the operator's boss. DRAGAT said that she notified her insurance and the owner PADILLAGONZALES. DRAGAT said PADILLAGONZALES failed to give more information regarding the incident and DRAGAT's insurance advised her to file a police report. DRAGAT said that her insurance and PADILLAGONZALES's insurance have been in contact with one another. I attempted to call PADILLAGONZALES with negative results. DRAGAT is going to update her insurance. No further incident to report.

I will follow up on my next tour. The witness, identified as Ms. Michelle Temple, stated that she observed the collision, and added that it was with enough force to cause damage to the bumper. She stated that she didn't believe any damage was caused, thus she didn't notify anyone.

No one was injured, and vehicle 1 did not need to be towed. On Monday, April 29th 2019, at approximately 1600 hours, I left a voicemail for the owner of Splash, Jamie Mcmanus, in regards to any possible surveillance footage. I requested that he call Newton Police headquarters and ask for me, when he gets the message.

UPDATE: Waltham PD was requested to check again for this vehicle at it's registered address tonight, 5/1/2019. This check resulted in negative results for the vehicle. No citation will be issued at this time until there is verification that this vehicle was indeed involved in this accident.

I responded to the area 107 Dartmouth St. at approx. 0345 on 04/28/9019 along with N493 Officer Kelly Donahue and N508 Sgt. Devine for a report of an MVA. The MVA was reported to be single vehicle crash. The vehicle left the roadway and was on the front lawn of 107 Dartmouth St. I arrived on location (first on scene ). I observed tire tread marks across the front lawn of 107 Dartmouth St. I then observed a vehicle in the bushes toward the left front side of the house. The vehicle; NY Reg. JHE8252, Porsche color blue. The RMV info on the plate came back registered to Porsche;of ;South. Registration plate type: Dealer Assignment. 185 W Sunrise HWY; Freeport,NY (no further info). The vehicle was unoccupied and secured. I felt the hood of the car which was still warm from the engine. I checked the immediate area surrounding the crash scene for an operator but found no one. I radioed to other responding units to canvas the neighborhood and surrounding area for the operator on foot. I spoke to the homeowners at 107 Dartmouth St., identified as Anthony and June Limberis. Mrs. Limberis stated she heard a load noise and a car engine revving. She stated when she looked out the window she observed a W/M tall, thin, wearing tan khakis and a light colored plaid shirt walking across the front lawn. He proceeded up Dartmouth St. (northbound) toward Commonwealth Ave. Newton Fire Dept arrived on location to check the crash scene and vehicle for possible leaking fluids and any other hazards. As I was talking with the firefighters we were approached by two men. One of the men was intoxicated. He was unsteady on his feet, had glassy bloodshot eyes, and was slurring his speech. The man was a W/M tall/thin approx. 6'2" and was wearing tan khakis and a light colored plaid shirt. He was identified as Eric Adams DOB 09/18/1969. The second man with him was identified as John Adams DOB 02/07/1936; An elderly W/M approx. 5'6" also wearing tan khakis and a light colored plaid shirt. Eric Adams stated his father John was the operator of the vehicle. Eric Adams had a strong odor of Alcoholic beverage emanating from his person and breath. He continued to slur his speech and was unsteady on his feet as he spoke. He was swaying back and forth and side to side as he spoke to me. Officer K. Donahue and Sgt. Devine were on scene at this time and also made the same observations. Multiple times we had to grab hold of Eric Adams to keep him from losing balance and falling. We spoke with Eric and John Adams for several minutes as their accounts of what took place were not consistent. I asked Eric Adams where the keys to the vehicle were. Eric Adams immediately pulled the starter key to the vehicle from his front right pocket. Eric Adams became defensive and belligerent when questioned about who was operating the vehicle. John Adams appeared tired (as if he just woke up) and confused as to the situation. Sgt Devine and I both spoke with Eric Adams and John Adams. Although both men claim John Adams was the operator of the vehicle, a clear determination was uncertain. A query of both mens driver's license status on NY Licenses came back active. Eric Adams has two prior DUI's on his RMV record. Officer Donahue and Sgt. Devine escorted the two men to 93 Dartmouth St. Eric Adams stated they were staying at that residence which is his sister's house. I stayed at the crash scene to document information and assist with clearing the MV from the property with Tody's Towing Service. As they were walking away I observed Officer Donahue and Sgt. Devine continue to have to assist Eric Adams with staying on his feet. Approximately ten minutes later as I was speaking with the aforementioned homeowners Mr.&Mrs. Limberis. Officer Donahue and Sgt. Devine returned to the crash scene. As they were updating me we observed Eric Adams returning to the crash scene. He was stumbling and swaying as he walked and repeatedly almost fell. Officer Donahue and Sgt. Devine immediately intercepted and assisted him before he fell. As they were assisting him again I continued speaking with the homeowners. Suddenly I heard Officer Donahue and Sgt. Devine struggling with Eric Adams who was now on the ground and resisting being handcuffed. I ran to assist them. Eric Adams continued to resist and be uncooperative until he was appropriately restrained in handcuffs. Eric Adams was placed under arrest by officer Donahue for Disorderly Conduct and Resisting Arrest. Reference Officer Donahues Newton Police Incident Report #19016484. The vehicle NY Reg. JHE8252 was removed from the property of 107 Dartmouth St. and towed from the crash scene by Tody's Towing Service.

On 6/13/19 I responded to 19 Sutcliffe Pk for a report of a MVA. Upon arrival I spoke with the operator of MV#1 who stated he turned onto Sutcliffe Pk to turn around and when he came back down the hill his brakes wouldn't' work when he struck MV#2 which was legally parked on the street. The owner of MV#2 was on scene and was not inside the vehicle at the time. There was heavy rear end damage and the owner stated he would call for his own tow first thing in the morning. On 6/28/2019, I responded to area of 350 Chestnut St for a a motor vehicle accident. The operator of MV1 ( Ma. Reg: DEB5) stated that they were stopped in traffic southbound on Chestnut St. MV1 stated that they were then rear ended by MV2 while they were stopped in traffic. The operator of MV2 (Ma Reg: S81436) stated that he was stopped in traffic southbound on Chestnut St behind MV1. MV2 stated that he saw the light at Commonwealth Ave turn green and started accelerating. MV2 stated that he collided with the rear end of MV1. All parties involved were checked by medics and signed patient refusals. No vehicle needed to be towed. MV1 sustained damage to the rear. MV2 sustainted no damage.

On 06/28/2019, while assigned to West Newton, I, Officer Conary, met with Owner of MV1 for a report of a past hit and run accident. Owner of MV1 explained to me that at approximately 0800 hours they parked MV1 on Walker Street on the side of the road. At 1700 hours, they observed a dent on front right bumper. There was a note left that stated "a green van hit your car and scraped - License plate C49807". There was no additional information. The registration comes back to a party out of Somerville MA, Frank Sarno Jr. Somerville PD was contacted to check the residence. They left a note to have Sarno call Newton Police. Investigation is still ongoing until all parties involved have been contacted.

On 01/03/2020, while assigned to WN, I, Officer Conary, met with reporting party WONG, Douglas for a report of a hit and run. WONG stated that he parked in his car, MV1, in the back of 330 Nevada Street and went to work. When he left work to return back to his vehicle he saw that there was damage to the left side and back tail light. WONG was able to retrieve the video for the parking lot and get the other vehicles information. WONG showed me the video and I saw that a white pick-up/trailer truck, MV2, was making a delivery and pulled out of the parking spot right next to MV1. After MV2 left the area the damage to MV1 was visible. I was able to get the plate information, and contact the company. I left a voicemail for them to call Newton Police regarding the accident. WONG was able to drive the vehicle. Video of the incident will be submitted to IT accordingly. No further incident to report. Traffic Bureau update (Officer Gaudet): I contacted Igos Welding Supply Company and spoke with the operator of MV2, Tyler Igo (S35470305). Igo stated he was operating MA CON: P90238 while making a delivery at 330 Nevada Street, Newton at the time the crash allegedly occurred. Igo stated he did not know his vehicle was involved in a crash and has not observed any new damage to his vehicle. Igo's information has been added to this report to reflect him as the operator of MV2. No further action will be taken.

On January 9, 2020 at apprrox. 1430 hours I responded to the parking lot of 333 Nahanton St " JCC greater Boston" for a report of a past hit/run. Upon arrival I met with the RP Mr. Eugene Magier d.o.b. 7-7-61 35 Brandeis RD Newton MA 02459. He stated he parked his vehicle in the parking lot of 333 Nahanton St JCC building at approx. 0930 hours. He stated he returned to his vehicle at approx. 1030 there was a vehicle next to him parked too close so he went through the passenger door to get into his vehicle. He stated when he went to his next destination, he exited his vehicle and observed the damage. I observed the damage to the driver side rear near the wheel well to the pass door. He also stated he went back to the parking lot where he parked and found a blinker light next to where he parked and the color of the blinker light matched the partial color of the damage to his vehicle. The blinker had "Toyota" on it and the following partial numbers "KOITO - 48-5 SAE EAP2 96" I canvassed the area and observed camera in the possible area of the crash. I went to the JCC and met with Director of facilities Brian Morris 617-558-6552. We observed some video in that area that may be visible if the vehicle goes against the building to park. Mr. Morris is going to watch the video to see if there is a vehicle within the time frame and can observe the crash. He will contact NPD of his findings. I advised the owner of the process. I then left without further incident.

The owner of MV#1 stated she had parked her vehicle in the parking lot of 1314 Centre St and went into the store (Holly Cleaners) to pick up her dry cleaning. The owner of MV#1 stated her friend "Rosemary" who was sitting in the front passenger seat of MV#1, observed the vehicle parked next to it back out of its parking spot and struck MV#1. I spoke to the manager of Holly Cleaners and she was able to provide me with the customer's contact information that drove MV#2. The manager of Holly Cleaners was able to show me video footages of the accident. The video shows that operator of MV#2 attempted to back out of his parking spot making a sharp turn and leaving the area. The video then shows the passenger of MV#1 get out of the vehicle and looked at the damages. I am unable to determined if contact was made between the two vehicles but it was highly possible that contact was made. MV#2 appeared to be a gold colored "Audi" SUV operated by a Fred Ellis of 36 Sallbury Rd in Brookline MA (617 731-3358). I am unable to view a registration or locate a registered vehicle to a Fred Ellis at this time. The damage to MV#1 is to its driver's side rear door. It displayed a light color mark (Minor damages). There were no reported injuries to all parties involved. The front passenger of MV#1 was only known as "Rosemary" (Unknown last name, address or D.O.B). I contacted the operator of MV#2 and left a voicemail. To be further investigated pending statements from the operator of MV#2. On 01/17/2020 at 09:45 I spoke to Alfred Ellis and he stated that he was at the Holly Cleaners parking lot and could not reverse due to MV#1 parking so close to his vehicle. Mr. Ellis stated he unsure if he had made contact with MV#1. Mr. Ellis was forthcoming with his statements and provided me with his vehicle and license information. He stated he was not injured and I provided him with this accident report number along with information of the other involved vehicle.

Owner of V1 stated while shopping, Ruth (HomeGoods employee), called her plate number over the speakers in the store. Ruth told the Owner of V1 she just witnessed a MVA involving her car. Ruth took a photo of the involved vehicle and watched V2 leave. Owner of V1 came and explained what happened to me. Ruth sent the photo to the Owner of V1. V1 sustained damage on the left rear side of the vehicle. I spoke to Ruth, she stated she watched V2 drive in, attempt to turn left into a spot, hit the vehicle, and leave right away. Ruth stated she took a photo of V2's plate and then went to the store to call it over the speakers. Ruth stated at no time did V2 stop. Ruth spoke to the owner and explained what happened. During the investigation, I spoke to the Opr of V2. She stated she was in the parking lot, but does not remember hitting a vehicle. Opr of V2 gave me all necessary information and apologized. V2 has minor damage on the right front side consistent with the damage of V1. V2 will not be charged. No injuries.

Vehicle #1 was traveling south on Crafts St. Vehicle #1 came to a stop to take a left turn into the operators driveway in front of 297 Crafts St. when it was struck in its rear end by Vehicle #2. Vehicle #2 was traveling south on Crafts St. and failed to slow for the Vehicle #1 that was stopped in traffic. Vehicle #2 crashed into vehicle #1 then continued south on Crafts St. before turning left onto Linwood Ave and leaving the scene. While on a routine traffic stop at California and Bridge St. Officer Spinney and Sergeant Mcneil heard me give out the description of vehicle #2. Officer Spinney and Sgt. Mcneil saw a damaged vehicle matching the description pass them going east on California St. Sgt. Macneil stated the truck said "Franchi" on the side. When Officer Spinney cleared the MV stop he was able to locate vehicle #2 located behind 37 Riverdale Ave. The Trucks hood and engine was still warm to the touch and had corresponding damage that matched witness statements. I was able to match pieces of the truck left at the scene of the accident to the vehicle Officer Spinney located. Pictures of the scene and both vehicles were taken and turned into the IT bureau. The co-owner of Vehicle #2 Robert Franchi could not tell us who was operating the vehicle. The co-owner of Vehicle #2 Robert appeared to be intoxicated and admitted to drinking. The owner of vehicle #2 has video footage of the vehicle being parked but could not provide us with a copy of it. Vehicle #1 suffered minor damage to its rear but was disabled and needed to be towed from the scene. Todys towed the vehicle. A towed MV report was completed and filed. Vehicle #2 suffered heavy front end damage primarily to its driver side. No injuries were reported.

Inga Sipicina stated that at 1050 hrs a brown UPS truck bearing S79977 struck a sign that was attached to the building above the entrance in two different locations. Inga stated the truck then took off. The sign had clear but minimal damage. A branch manager from UPS later came to the store to speak with Inga and her husband Slava and stated they would be in contact with them but didn't leave any information. MV#1 was parked in a stall of the parking lot of 199 Boylston Street when it was struck by MV#2. The owner and operator of MV#2 left the area, however, during an investigation of the mater the owner and driver was identified and located. -OpMv#1 stated that approximately 1140 she parked her vehicle in a parking stall in the parking lot of 199 Boylston Street. She stated she returned, approximately, an hour later and discovered the damage to her vehicle. -OpMv#2 stated, via telephone, she was at 199 Boylston Street, was operating MV#2, however,that she did not know she had struck Mv#2. She stated it was a very tight parking space but did park in the stall. - While at the scene I observed a scrape on the door of MV#1 front right passenger door. I next observed that MV#2 very close to MV#1 making it very difficult to move between vehicles. I also observed that MV#2 was parked very far forward than the vehicle on the right. I observed a scrape on the front bumper with traces of white paint into it; it appeared to be paint transference. -Neither vehicle was towed from the scene and since neither driver was in the vehicle at the time of the collision there were no injuries reported.

He stated he found that s.t.b. yesterday 21:00 and 05:00 this morning, a vehicle had crashed into his fence and left the area. On he Circuit Ave side of his residence, I observed a post and two sections of white vinyl fence that were knocked over and damaged. One section of the fence had been propped back up and had tire marks on it. There was no other damage to the area or utility pole (located right near the fence). On February 10, 2020 at approximately 1911 hours, while on patrol in Newton Centre Sq, I was approached by a woman who wished to report a past MV hit and run. Ms. Eileen Kavall stated last Thursday(02/06) between the hours of 1745 and 1830, her MV was parked in a handicap spot in front of the Star Market at 1 Boylston St. While parking her vehicle, she noticed a large white SUV parked in the spot directly next to her. She stated this vehicle was also in a handicap parking space but did not see a handicap sticker on the vehicle. When Eileen exited the store, she observed damage to her front left fender and the SUV was gone. I also observed this damage. Eileen did not have a number plate for the MV that hit her but stated the Star Market may have cameras facing where the incident occurred. I then provided Eileen with an accident report number and went to Star market to speak with the manager. At Star Market, I spoke with Janet Yaksic, store manager, who stated she remembered speaking with Eileen about the accident. Janet stated that she would have to contact the store's director in order to contact the loss prevention employees who do not work on site. The loss prevention employees would be able to review any possible footage by the end of the week and contact Newton Police with their findings. I left my name and contact info with Janet. This accident is still under investigation pending a review of the Star Market footage. Update: 02/22/20 I have not received any video surveillance from Star Market. RP will be advised to pursue with their insurance company.

On Wednesday 02/19/2020, at approximately 1500 hrs, operator of MV1 stated she was traveling eastbound on Newtonville Ave, right before Walnut Street, a dark colored pick up truck was traveling behind her. The operator of vehicle 1 stated she was traveling West on the Washington Street bridge over the Mass Pike when vehicle 2, a gray SUV attempted to change lanes, as this happened the front passenger side quarter panel of vehicle 2 came in contact with the rear drivers side quarter panel of vehicle 1. This caused minor scratches to vehicle 1. The operator of vehicle 1 attempted to pull over and exchange information with the other driver. The operator of vehicle 2 continued driving away and got onto the Mass Pike, during this process the operator of vehicle 1 and the passenger of vehicle 1 were able to get the license plate number of vehicle 2 which was NY REG:T797800C. In an attempt to locate and identify the driver of vehicle 2 I ran NY REG T797800C which comes back registered to a short term rental company called Flexdrive. I called a phone number on their website 8663539399 which is listed as the number to call if you are in an accident with one of their vehicles. I was only able to speak to a service technician, a Tom Louis of ARI Fleet Service 18002272273, who told me the vehicle was out of the Flexdrive North Dighton Mass hub located at 123 Williams Street North Dighton MA 02764.Tom also mentioned the location would most likely not be open again until Monday. He did not have any other information regarding who may be operating the vehicle. I have attempted to find contact information on this North Dighton location which I have had negative results so far. I have also emailed the Flexdrive customer service department. To be followed up on. Traffic Bureau update (Officer Gaudet): On Tuesday, February 25, 2020, I made contact with the registered owner of MV2, Flexdrive. The representative from Flexdrive stated MV2 is registered to the company but Lyft uses their vehicles for ride sharing. I contacted Lyft and spoke with a party named Jonas from their support team. Jonas stated he would make contact with the operator of MV2 and ask them to contact me to give a statement. Lyft stated they would also conduct their own internal investigation. On Wednesday, February 26, 2020, I spoke with the operator of MV2, Gary Procaccini (S36066201). Procaccini stated he was operating a 2019 Santa Fe (NY Reg: T797800C) as a Lyft driver on the evening of February 22, 2020. Procaccini stated he was traveling on Washington Street (W) towards the Massachusetts Turnpike entrance in West Newton. Procaccini stated he was in the wrong lane and looked in his mirror before attempting to merge into the correct lane. Procaccini stated to the right of his vehicle he observed a vehicle "flying" towards the entrance to the Pike. Procaccini stated he came to stop to allow the vehicle to pass. Procaccini stated the vehicle passed at a high rate of speed and he continued on his way onto the Pike (E). Procaccini stated he had a passenger in the vehicle using Lyft servies and there were no signs that a crash had taken place. Procaccini's information has been added to his report to reflect him being the operator of MV2. I then spoke with Peter Houser who is a Driver Services Representative with Lyft. Houser stated he had just visually inspected the vehicle alleged to have been involved in the crash. Houser stated the vehicle was in "pristine" condition and he observed no damage to the front passenger side quarter panel of MV2. Houser e-mailed me pictures of the vehicle. I observed no damage to MV2. Those pictures were submitted to the IT Bureau.

Mv#1 owner stated he parked his Mv on Lakewood Rd (opposite #23 driveway) at about 1000hrs this date. Owner stated he returned to his unoccupied parked Mv at about 1045hrs to find fresh light moderate driver's side front end damage with light scrape marks. On 5/22/20 at approx 1245 I responded to the Dunkin Donuts for a report of a MVA Hit and Run. Upon arrival I spoke with the RP and vehicle owner Diana Santiago who stated she was working inside Dunkin Donuts when one of her employees told her that her car was just struck. Ms. Santiago came out and approached the operator of MV#1 who doesn't speak english well but stated she didn't hit the car, and then left the area. I spoke with the registered owner of MV#1 who stated her mother-in-law was driving the vehicle and that she lives on Parker St. Ofc. Gabriel went over to speak with her and confirmed the vehicle was involved in an accident with rear end damage. Ms. Girginis stated she didn't even realize she was involved in an accident. MV#2 was parked on Langley Rd as MV#1 was backing up to get out of her parking spot and struck MV#2. There was light damage and no injuries to report.

On June 14, 2020 at approximately 17:13 hours Newton Police responded to Union St at Langley Rd for a report of a motor vehicle crash into bicyclist where the motorist took off. Upon arrival, I spoke with the operator of Bike1, who informed me that he was traveling eastbound on Union St when he rode his bicycle into the back of MV1. MV1 did not get out of the vehicle and drove away from the scene. Bike1 informed me that he was traveling behind MV1 and observed MV1 traveling slow on Union St at approximately 10-15mph. MV1 stopped at 45 Union St which is when he collided into the rear bumper. Witness 1 said that she saw a white male operator of MV1 and a female passenger. Both the operator and passenger looked behind them and then continued to drive away. it is unknown if the operator was aware that there was a collision. It should be noted that the collision caused a loud yell and bang that multiple people were able to hear. Bike1 did not have any visible damage to his bicycle and declined medical attention. A photograph of MV1 was forwarded to IT to be attached to this report along with a photograph of bike 1.

V1 operator stated both V1 and V2 were traveling westbound on Washington St. V1 operator alleges V2 made a left turn, from the outside lane (double lane) attempting to turn left into 2300-2310 Washington St. V2 crossed directly in front of V1. The V1 operator stated he could not avoid a collision. V1 collided into V2 at an angle causing V2 to spin out, facing eastbound on the eastbound side of Washington St. V2 operator stated he was travelling westbound on Washington St. in the inside lane. V2 operator alleges he slowed with a left turn directional activated and was suddenly rear ended. I observed the damage to both vehicles. I could not determine based on the damage, that I observed, who's account of the crash was consistent with the damage. I advised both operators that i would be documenting both of their accounts of the MVA.
