MV1 (MA Reg: 63EP17, a 2000 Acura 32TL; operator: Carlos Standford) was traveling NORTH on Cherry St when a vehicle swirved into his lane head-on. In order to avoid MV3 he swirved into the other lane and side-swiped MV2. MV2 (MA Reg: 9JX388, a 2019 Subaru Outback; operator: James Cant) was traveling SOUTH on Cherry St when he had come to a complete stop, put his left turn signal on to show him turning left into his driveway at 176 Cherry St. He was waiting to make his turn until MV1 had driven past so it was safe to do so. As he was at a complete stop, MV1 collided with his rear driver side bumper in an effort to avoid a head on collision with MV3. MV3 (MA Reg: 93YE85; a 1997 Toyota Camry; operator Matthew Kuniholm) was traveling SOUTH on Cherry St when he had seen MV2 at a complete stop with a turn signal. He didn't give himself enough time to stop, began to slide as he was braking and went over the marked lanes into oncoming traffic in an effort to not collide with MV2. MV3 operator, Matthew Kuniholm, was issued in hand MA Uniform Citation T1440470 for Marked Lane Violation 89/9a. All vehicles were still in driveable condition. MV1 had damage to the front driveside wheel fender and driver door. MV2 had slight damage to rear driver side bumper. MV3 did not have damage to his vehicle. Weather was icy and there was an active snow fall during the time of the accident, which was 1815hrs.

On the above date/time I responded to Washington Street at St. James for a two motor vehicle crash. Upon arrival with the operator of vehicle one stated he was stopped at the red traffic signal and was struck from behind. I spoke with the operator of vehicle two who stated that she was traveling on Washington Street she applied her brakes to slow for the red traffic signal. She stated when she applied her brakes due to the decline icy roadway her vehicle slid into vehicle one. Passenger in vehicle two stated the same as operator two. Due to the damage on vehicle two was towed by Tody's towing of 1354 Washington Street 617-527-0013 to their tow lot. No injuries all parties advised.

On the above date/time I responded to the area 2000 Washington Street Newton -Wellesley - Hospital for a two motor vehicle crash on the 3rd floor of the parking garage. Upon arrival I met with the operator of vehicle one, she stated she was pulling into a parking spot and applied the brakes but due to the ice on the roadway and slope of the roadway her vehicle slid into the rear of vehicle two which was parked. Vehicle two was unoccupied, and dispatch tried to get in contact with the owner to advise them. I also left a message with NWH in case the owner comes back in. No injuries and I advised of the process. Later in the day the owner vehicle two came to NPD, I gave him vehicle one inforamtion and advised him of the process.

On 01/22/19 at approximately 14:25 I responded to Winchester St near Curtis St. for a report of a hit and run accident. Upon arrival I spoke with the operator of veh #1 who stated he was driving due West bound on Winchester St when a truck (White 3/4 Ton Pick up with plow in front and sander in back) turned left onto Winchester St from Boylston St. Veh #2 caught his plow on the drivers side rear passenger door and rear quarter panel of veh #1. The crash pushed veh #1 into the opposite lane for a moment until he could turn back onto his side. The operator of veh #2 pulled over and got out and said "you didn't see me?" and got back into his truck and took off down Winchester St West bound. The operator of veh #2 is described as a Hispanic male 18-25yrs old, 5-8'-5'11", lean build, clean shaven, yellow jacket with black hair. No license plate was ever obtained. Veh #1 was towed by Todys Towing and owner of veh #1 was given a ride home to Needham.

On Tuesday January 22nd 2019 at approximately 1615 hours I responded to a two car motor vehicle accident at the intersection of Beacon Street and Crescent Ave, both are public ways in the City of Newton. At the time of the accident the current weather conditions were clear and dry with the road conditions extremely icy and snow covered. Upon arrival the medics were evaluating the operator of MV1 for slight back pain. The operator of MV1 signed a patient refusal and stated she was headed westbound on Beacon Street and stopped at the intersection of Crescent Ave, attempting to turn left. The operator of MV1 stated she came to a complete stop and had her left turn signal on waiting to turn onto Crescent Ave. She states MV2 then rear ended her and she pulled over the side of the road to wait for police assistance. MV1 was completely disabled with major rear end damage. MV1 was towed by Todys. The operator of MV2 stated he was traveling behind MV1 and attempted to stop behind MV1 but due to the icy road conditions, MV2 failed to stop and collided with MV1. The operator of MV2 refused medical attention and was able to drive MV2 from the scene.

At approximately 1615HRs. on Thursday, January 24, 2019, I was dispatched to the intersection of Elliot St. and Boylston St. for a report of an MVA. Upon arrival, I located the operator of MV1 (MA Pass: 1NB459) who had pulled into the BP gas station. He stated that he was traveling Northbound on Elliot St. when his traffic signal turned red and he brought his vehicle to a complete stop at the intersection. He stated that at this time, he was rear ended by MV2. He and the operator of MV2 then got out of their respective vehicles and spoke about the collision, after a few moments, the operator of MV2 got in her vehicle and left. Using the picture of MV2's license plate (MA Pass: 3EY477) which he provided me, I was able to contact the operator of MV2 at her residence. She stated that she was traveling Northbound on Elliot St. and observed MV1 come to a stop and she applied her brakes and came to a complete stop aswell, without making any contact to MV1. She further stated that the operator of MV1 became agitated and started to yell at her. She then left because she asserts that there was no collision and she was concerned by the operator of MV1's behavior. I observed no damage to MV1's rear bumper area, with the exception of an old dent, approx. the size of a quarter, which was highlighted with some type of permanent marker. I observed no damage to MV2's front bumper area and I noted large, pronounced license plate bolts affixing MV2's license plate to its bumper. The operator of MV1 stated that he was suffering from back pain due to the collision and elected to drive himself to Carney Hospital. Due to there being no damage to either vehicle and the large plate bolts on MV2, which would have likely caused notable damage to MV1's bumper, I will not be charging the operator of MV2 with leaving the scene of an accident at this time. Both parties have been advised to contact their insurance companies and I cleared without incident.

On January 25, 2019 at approximately 17:46 I responded to a motor vehicle hit and run. It was reported that a white pick-up truck crashed into a parked car and left the scene. I checked the area and was unable to find a vehicle matching the description. I responded to the crash location and spoke with the owner of MV1, who was not with his vehicle when it was struck. MV1 was parked legally on the shoulder of by 1743 Beacon St. The vehicle that struck MV1 left the travel lane and struck MV1 in the driver's side rear, causing damage. I spoke with the reporting party, Tim Leary, who heard the crash and saw a white Ford F-150 leaving the scene with damage on front passenger side. Leary did not see the license plate of the truck but thinks it was the 2006-2012 body style. The owner of MV1 was going to arrange for a private tow at a later time.

On 01/28/2019 Karen Griffin came into NPD Headquarters to report the following. Griffin states that on 01/17/2019 her vehicle (V1) was parked in a parking space in the lower parking lot of Wegman's 200 Boylston Street. Griffin states that V1 was backed into a parking stall near the entrance of the parking garage. Griffin states that between 1015 and 1130hrs V1 was stuck by an unknown motor vehicle (V2). V2 left the scene without leaving leaving their information. At the time of the collision V1 was parked and unoccupied. As a result of the impact V1 sustained minor damage to the passenger side front bumper. Griffin states that an employee of Wegman's witnessed the crash and described V2 as a Gray Honda Accord with a possible Maryland Reg JNK824. I conducted an RMV check of V@ registration with negative results. At this time the identity of V2 is unknown. No injuries were reported. Griffin stated she did not report the crash to the police until instructed to do so by her insurance company.

Operator of MV1 (Mass. reg 4458XR) states he was traveling east bound on Commonwealth Ave when he approached the intersection. He states he had a flashing yellow light, slowed down and proceeded through the intersection when he collided with MV2. Operator of MV2 (CT reg. AA22048) states she was traveling north bound on Walnut Street when she approached the intersection. She states her light was solid green and slowed down as she proceeded through the intersection because MV1 was also proceeding through the intersection. MV2 and MV1 collided. When on scene I observed the traffic lights to not be working properly. The north bound traffic lights on Walnut Street were completely out and at times would flash red. The Commonwealth Ave lights were flashing yellow. It appears the cause of the accident is a faulty traffic light. Dagle Electric had to come out to the crash scene to re set the faulty lights.

CALLER WHO REPORTED THE CRASH DID NOT REMAIN ON SCENE. HE REPORTED THE OFFENDING VEHICLE WAS A SILVER VAN DRIVEN BY AN ASIAN MALE HEADING W-BOUND ON WATERTOWN ST. Operator of MV1 states he was traveling eastbound on Lagrange St approaching Bryon Rd when MV2 made an "unusual" left turn attempting to travel southbound on Bryon Rd. Operator of MV1 states when MV2 made the left turn, MV2 collided with MV1 causing damage to the left front corner panel and drivers side door. Operator of MV2 states while making the left turn, MV1 looked like it attempted to come to a stop but slid due to the snow and slippery road conditions. Operator of MV2 states his tires spun out attempting to make the turn and struck MV1 with the front left corner panel causing significant damage. It should be noted the weather during this time was heavy snow with wet and slippery road conditions, which is a determining factor on how the accident happened.

On 2/12/2018 at approx 1445hrs while assigned to 497 I responded to the area of 659 Chestnut St for a report of a two car crash . Upon arrival Ma Reg 9VA441 being operated by Steven HUTCHINSON and Ma Reg 98NL90 being operated by Vi DANG were pulled to the side of the road, there was a heavy snowfall and the road had accumulated a thick layer of snow and multiple crashes were occurring. NFD was on scene , DANG was crying and hysterical despite the minor nature of the crash and zero damage to either vehicle. HUTCHINSON stated they were creeping along NB on Chestnut St when DANG stopped, HUTCHINSON attempted to stop and his 4wd engaged and he slid slightly into the back of DANGS car, no marks or damage to either car. DANG elected to go to NWH with Cataldo . DANGs veh towed so as not to obstruct roadway.

The operator was driving northbound on Woodland rd and was taking a left hand turn on to Maple st when the car slipped on the snow/ice and collided with the no parking sign on the sidewalk. No injuries reported. Todys was notified to help assist getting the vehicle off of the sign. Two pictures were taken and submitted. The city was notified of the sign being down and in need of a replacement.

The operator of the motor vehicle stated he was driving northbound on Chestnut st at approximately 15 miles per hour due to the icy conditions and felt his car starting to slip down the hill. He stated that the car lost control and spun out onto the sidewalk and rested up against a utility pole. The drivers side door was caved in and the operator got out through the back seat passenger door. There were no injuries reported. The vehicle was towed by Todys. The city was notified of the black ice.

On Thursday, February 21, 2019, while assigned to Traffic unit N526, I responded to the area of Ionia Street, Newton for a report of a motor vehicle accident involving a City of Newton owned vehicle. Ionia Street is a public way maintained by the City of Newton. The weather at the time of the accident was overcast. The road surface was icy and snow covered. The operator of MV1, Patrick Carresi (MA: S73958042), stated he was operating a City of Newton owned 2016 Ford F350 plow truck (MA MVN: M94756) turning left onto Ionia Street from Auburndale Av. Carresi stated as he took the turn, the rear passenger corner of the truck made contact with the drivers side front panel of MV2 which was parked on the side of the road facing southbound. I observed minor damage to the rear of MV1. MV2 received heavy front end damage with the entire front panel torn off. Carresi reported no injuries. Carresi works for the Utilities Department with the DPW. The operator of MV2, Donna Canavan (MA: S57637058), stated she was parked inside her 2005 Honda CRV (MA: 8ZEE40) facing southbound on Ionia Street. Canavan stated as the plow truck took the turn onto Ionia Street from Auburndale Av its rear corner of the truck struck the front panel of her parked vehicle ripping the panel off. I observed significant damage to the front panel area of MV2. Canavan reported no injuries and was able to drive her vehicle from the scene. Pictures were taken of both vehicles and submitted to the IT Bureau.

The operator of MV#1 stated she was travelling southbound on Walnut St at the intersection of Elm Rd when she let a vehicle out of Elm Rd to make a left turn. The operator of MV#1 stated she proceeded to move forward when a school bus directly behind that vehicle also made a left turn making contact with MV#1. The operator of MV#1 honked her horn to get the school bus's attention with negative results. MV#1 sustained minor damages to its driver's side front bumper and fog light. The damage to the school bus should be to its driver's side rear bumper. There were no reported injuries to the operator of MV#1. There were no reports of any contracted school bus operators reporting an accident. The operator of MV#1 stated there were witnesses at the accident scene but she did not get any of their information. The operator of MV#1 was advised to contact her insurance provider.

VEHICLE #2 STILL WAS FORCED INTO THE REAR OF VEHICLE #1 BY VEHICLE #3. OPERATOR #3 STATED HE WAS GOING N/B ON WASHINGTON ST WHEN HE ATTEMPTED TO GO AROUND VEHICLE #2 ON THE LEFT THINKING THERE WAS ENOUGH CLEARANCE FOR TWO LANES OF TRAFFIC. AT THE TIME PRIOR TO CRASH THERE WAS A SINGLE LANE OF TRAFFIC AHEAD. OPERATOR #3 FURTHER STATED AS HE BEGAN TO GO TO THE LEFT OF VEHICLE #2 HE THOUGHT SHE BEGAN TO GO TO THE LEFT ALSO CAUSING HIM TO HIT HER VEHICLE. I AGAIN SPOKE WITH OPERATOR#2 ABOUT WHAT OPERATOR TO VEHICLE #3 STATED AND SHE STATED THAT SHE NEVER WENT TO HER LEFT PRIOR TO THE CRASH.

The operator of MV1 stated that he was making a delivery at Frame It and double parked MV1 facing West bound in front of 1645 Beacon Street. The operator of MV1 then stated that we he left Frame It and went into another store MV1 was still parked out front. When he came out moments later MV1 was not where it had been parked and had rolled across the street crashing into MV2 which was parked facing east bound on Beacon Street. MV1 sustained minor damage to rear bumper while MV2 sustained major damage to the rear bumper. Neither operator was involved in the crash so medics were not notified. MV1 left the scene driven by its operator and MV2 was towed by Todys. The operator of MV2 was not on scene and unreachable at the time of this report.

Operator of vehicle one ( MA reg 3986BV 2012 MERZ ) , Vladimir Krapchev stated that on 03/21/2018 at 08:56 hours he was stopped at a red light on Center St at Commonwealth Ave. Krapchev stated that his vehicle was heading south on Center St and was the third vehicle in line to turn west bound on Commonwealth Ave. Center St and Commonwealth Ave are both public ways in the city of Newton. Krapchev stated that " a red/maroon full size sedan fire department car " struck the front driver's side fender of his vehicle. Krapchev stated that the fire department car was travailing south bound on Center St in the north bound lane, passing stopped south bound Center St traffic. Krapchev stated that there was also north bound traffic on Center St that was turning from the west bound lane of Commonwealth Ave. Krapchev stated that the fire department car turned onto the east bound lane of Commonwealth Ave and did not stop. Operator of vehicle two Newton Fire Department LT Phillip McCully stated that he was driving Newton Fire Department Car 14 ( MA reg MVN MF461 2004 Ford Crown Victoria ) south bound on Center St with his vehicle's emergency lights and siren activated. Lt McCully stated that he was responding to a motor vehicle crash with possible entrapment on Hammond Pond Parkway. Lt McCully stated that he was passing stopped Center St south bound traffic in the north bound lane of Center St. Lt McCully also stated that there was north bound traffic on Center St and " it was tight between the cars ". Lt McCully stated that he was unaware that a crash had occurred. Lt. McCully stated that he did not fell and contact between his vehicle and Krapchev's vehicle. Lt McCully also stated that he did not hear the crash because of his vehicle's siren was activated. Krapchev's vehicle had minor damage and red paint marks on it's front driver's side fender. The Fire Department car had scuff marks on it's passenger side rear door. Neither vehicle was towed and both operators stated that they were not injured in the crash. I took photos of both vehicles and forwarded the disk to the NPD It bureau for down loading.

I CHECKED WITH EMPLOYEES INSIDE THE VILLAGE BANK TO SEE IF THEIR SURVEILLANCE CAMERA OUTSIDE THE FRONT DOORS CAPTURED THE ACCIDENT, BUT THE VEHICLE WAS OUT OF VIEW. I WAS TOLD BY ONE OF THE EMPLOYEES THAT SOMEBODY FROM THE SECOND FLOOR OBSERVED A BLACK PICKUP TRUCK BACK UP INTO THE RP'S VEHICLE BUT DIDNT'T GET A PLATE NUMBER OR A DESCRIPTION OF THE DRIVER.

On Thursday, April 4th 2019, I responded to a one vehicle accident on Dedham St by Oak Hill St. The vehicle (Mass Commerical # 704 ADVA MIXER Color Red) was operated by Carlos Paz. His vehicle was headed west on Dedham St when the top of his vehicle struck a low hanging wire and dragged other wires to the point where it made utility pole # 104 tilt over toward the middle of the rd. There were no injuries. Eversource and Verizon responded to fix the pole. Carlos suffered no damage to his vehicle. Carlos has Travelers Insurance.

The operator of vehicle 1 stated that with a green left turn arrow showing, and her left directional on she went to turn onto Waltham Street from Crafts Street when all of the sudden she hit a bicyclist. She stated she did not see the bicyclist at all prior to him hitting her car. The bicyclist stated does not remember anything from the accident at this time. The bicyclist was traveling southbound on Crafts Street and appears to have entered the intersection of Crafts and Waltham Street without stopping and while a red light was showing. He was wearing a helmet at the time of the crash, he was not wearing any reflective clothing(dark green jacket), and the bike did not have any lights. The bike was taken to support services for safe keeping by Ofc. J.March. Vehicle#1 was towed away by Tody's towing due to extensive damage. Bicyclist was transported to Beth Israel hospital for several trauma injuries. Pictures of scene was taken by me and sent to IT Bureau. Both witness's statements were the same. They stated that they were behind vehicle 1 in traffic and saw that she had on her left dirrectional signal and had a green left arrow signal when vehicle 1 went to turn left onto Waltham Street.

Narrative:
On Wednesday, April 10, 2019 while assigned to Traffic unit N525, I responded to the intersection of Centre Street and Wesley Street, Newton for a report of a motor vehicle accident involving a bicycle. It was also reported that the parties involved were arguing with each other. The weather at the time of the accident was cloudy and the road surface was dry. Centre Street and Wesley Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Abdulaziz Stewart (S04075169). Stewart stated he was operating his 2017 Toyota Camry (MA: 669FM3) at the time of the accident. Stewart stated he came to a stop on Wesley Street (E) at Centre Street and was attempting to take a left turn onto Centre Street. Stewart stated there were no vehicles in the Southbound lane of Centre Street and there was a vehicle stopped in the Northbound lane of Centre Street motioning to him to take his left turn. Stewart stated as he began to start his left turn, a bicyclist crashed into the front passenger area of his vehicle. Stewart stated the bicycle had been traveling Northbound on Centre Street in the Southbound Lane. Stewart stated after the collision the bicyclist was yelling at him and called him a "Nigger". Stewart and his occupant, Miriam Stewart, reported no injuries. Miriam is a student at Bigelow Middle School. I observed minor damage to the front passenger quarter panel area of MV1. I spoke with the bicyclist involved in the accident, Timothy Murtagh (S49233464). Murtagh stated he was traveling Northbound on Centre Street on his Cannondale bicycle in the Southbound lane. Murtagh was not wearing any safety equipment while operating his bicycle (helmet) and was wearing a dark red jacket. Murtagh stated as he approached Wesley Street, MV1 "jammed on the gas" and hit him in the roadway. Murtagh stated he then fell to the ground and landed in the middle of the street. While speaking with Murtagh I told him it was reported to me by the operator of MV1 that he used a racial slur during their interaction. Murtagh acknowledged that he did and stated, "I called him a nigger". I observed moderate damage to the front wheel/rim of Murtagh's bicycle. Murtagh reported no injuries. Murtagh was advised and issued Massachusetts Uniform Citation T1443441 for Chapter 85, Section 11B (Fail to ride on the correct side of the roadway). Photos were taken at the scene and submitted to the IT Bureau. Due to the language used by Murtagh during his exchange with Stewart, Officer Rosenbaum of the Community Services Bureau was made aware of the incident for follow up.

The operator of MV#1 states that she was traveling north on Melrose St from Auburn St when she encountered a large flat bed truck blocking the roadway in front of #283 Melrose St, which is currently under construction. The operator of MV#1 states she then spoke with an individual whom she believes to be Steven Vona of Vona Construction who is currently developing the site at #283 Melrose St. According to operator #1, Mr. Vona told her she needed to turn around immediately, which she attempted to do when the front of her MV struck a raised sidewalk causing damage to the front of her MV. Operator #1 was very concerned that there was no signage or personnel from Vona Construction directing motorist where to go and feels he is responsible for the damage to her MV as he was the one informing her she needed to leave the "construction zone" immediately. She further states that no one assisted her in turning around, her view was obstructed because her MV is so low to the ground and that a construction truck was blocking the nearest driveway, making it even more difficult to maneuver. As I entered Melrose St from Comm Ave when I arrived I observed no signage and MV's were having to turn around. I informed Steven Vona that if he was going to temporarily block the street to unload equipment he needed to either use cones, signs or workers to assist drivers. In regards to the crash, Mr. Vona stated that as operator #1 attempted to turn her MV around the front of her MV struck the sidewalk. On Tuesday, April 16, 2019 while assigned to Traffic unit N525, I responded to the area of Plymouth Road and Cochituate Road, Newton for a report of a motor vehicle/pedestrian accident involving a City of Newton vehicle. The weather at the time of the accident was clear and sunny. The road surface was dry. Plymouth Road and Cochituate Road are both public ways maintained by the City of Newton. The injured pedestrian, Officer Steven Emmanuel, was transported to Newton Wellesley Hospital prior to my arrival on scene. I spoke with the operator of MV1, Officer Justin Lau. Lau stated he was on duty in his official capacity as a Newton Police Officer while operating a City of Newton owned 2018 Ford Fusion (MA: 9PYM80, Newton PD #1319). Lau stated he was parked on Plymouth Road (N) at Cochituate Road and was speaking with Officer Steven Emmanuel. Lau stated Emmanuel was standing next to the front driver side door area of his vehicle in the roadway. Lau stated a large delivery trucked approached their location on Plymouth Road from Woodward Street and was attempting to make a right turn onto Cochituate Road. Lau stated Emmanuel directed him to back his vehicle up so the delivery truck could get by their location. Lau stated he cut the wheel of the vehicle he was operating to the right; and very slowly began to back up from his location. Lau stated he was attempting to back the vehicle up to get closer to the curbing and out of the roadway. Lau stated as the vehicle was in motion the front driver side wheel made contact with Emmanuel's left foot. Emmanuel's left foot subsequently became lodged in the front driver side wheel well area. Emmanuel fell to the ground in obvious pain. Emmanuel directed Lau to pull the vehicle forward in an effort to release his foot. Emmanuel's foot was freed and NPD Dispatch was notified of the accident. Pictures were taken of MV1 and submitted to the IT Bureau. I observed no damage to the front driver tire area. I spoke with Officer Steve Emmanuel in the Newton Wellesley Hospital emergency room where he was being evaluated for a left foot injury. Emmanuel stated at the time of the accident he was working a paid detail for Paramount Pictures on Plymouth Road in his official capacity as a Newton Police Officer. Emmanuel stated he was speaking with Officer Lau on Plymouth Road at Cochituate Road. Emmanuel was standing in the street on his post and Lau was sitting in MV1 (parked). Emmanuel stated a Fed Ex delivery truck approached their location in the roadway and was attempting to get by them. Emmanuel stated Lau began to back his vehicle up in an effort to get out of the way of the delivery truck. Emmanuel stated the front driver side tire of MV1 ran over his left foot. After the tire ran over his foot, it then became lodged in the wheel well and he fell to the ground. Emmanuel stated his foot was freed from the vehicle and he was transported to Newton Wellesley Hospital by Newton Medics for further evaluation.

On Thursday 04/18/209 at approximately 1222 hours I responded to 81 Oak Street for a motor vehicle accident involving Newton Fire Ladder 2. At the time of the accident the weather was cloudy and road conditions were dry. Oak Street is a public way in the city of Newton. Upon arrival I spoke with the operator of MV1 who stated while trying to manuever through oncoming traffic on Oak Street heading southboud MV1 struck MV2's drivers side mirror while MV2 was parked. MV1 reported no damage and reported no injuries. MV2 reiterated the same events and reported very minor damage to the drivers side mirror. No injuries were reported. While attempting to pull MV1 back into the traffic lane he struck a fire hydrant knocking it over and completely detaching it from the base on the ground. As a result of the impact, vehicle 2 sustained extensive damage to the passenger side and had side air bag deployment. The passenger in vehicle 2 stated he did not see anything until impact. The Occupants of vehicle 2 stated they had no injuries or pain and were taken away from the scene by parent Niels Steenstrup. As a result of the accident vehicle 2 was towed by Tody's by Billy.

The operator of MV1, Irina Gorolevici (S18897891) stated she was stopped in traffic on Centre Street (N) at a red light while operating her 2017 Toyota Highlander (MA: 8JY370). Gorolevici stated while stopped in traffic MV2 crashed into the rear of her vehicle. I observed damage to the rear bumper area of MV1. Gorolevici reported no injuries and signed a patient refusal with Newton Medics. The operator of MV2, Wiranya Thatwisai Partosan (S27096641) stated she was stopped in traffic on Centre Street (N) at a red light while operating her 2016 Honda Civic (MA: 5VA815). Thatwisai Partosan stated while stopped in traffic MV3 crashed into the rear of her vehicle. The force from MV3 crashing into the rear of her vehicle caused MV2's front bumper area to crash into the rear bumper area of MV1. I observed moderate to the front and rear bumper areas of MV2. The vehicle was removed from the roadway by Tody's towing. Thatwisai Partosan reported no injuries and signed a patient refusal with Newton Medics. The operator of MV3, Sofija Garcevic (S74039706) stated she was traveling North on Centre Street towards Cabot Street. Garcevic stated she looked up and observed MV2 in front of her. Garcevic stated she did not have time to stop and crashed into the rear of MV2. I observed moderate to the front bumper area of MV3. The vehicle was removed from the roadway by Tody's towing. Garcevic reported no injuries and signed a patient refusal with Newton Medics. As a result of the accident, Garcevic was provided with Massachusetts Uniform Citation T1443443 in hand for Newton City Ordinance Chapter 19, Section 75 (Fail to use care, stopping).

The operator of MV#1 stated that prior to the crash she was traveling north on Rowe St. Operator #1 came to the intersection at Webster St, stopped and observed no other cars at the intersection and proceeded straight thru the intersection. While in the intersection, operator #1 observed MV#2 traveling west on Webster St travel thru the stop sign without stopping and strike her MV. MV#2 did not stop, but instead continued on Webster St towards Auburndale Square. Operator #1 describes MV#2 as a gold sedan being operated by a 55-70 year old female wearing large sunglasses. A search of the area was negative.

I responded to Brae Burn Country Club for a report of a past MVA involving a tractor trailer. I spoke to Tim Strano a manager for Brae Burn who stated that on Monday evening a truck and trailer struck the properties fence. The fence is described as a automatic gated security fence which opens and closes upon entry and departure. Strano provided me with the vehicles information which I proceeded to contact the owner and operator of the vehicle. I spoke to the owner/operator of Vehicle One who stated he received the wrong destination address and attempted to turn around in the rear of Woodland Train Station when he stuck the fence (Brae Burn CC Maintenance Area). He gave his contact information to one of the Brae Burn workers on scene and left the area. Both parties were given the crash information and were advised.

Operator of MV1 (Ma Reg 35T230) stated she was exiting the JCC parking lot, turning right onto Nahanton Street when a black SUV (No further description was provided), came around the corner, heading Southbound on Nahanton Street when MV2 (Black SUV) struck MV1's driver side front bumper. Operator of MV1 stated MV2 was traveling at a speed greater than it should around the corner prior to striking her vehicle. Operator of MV1 stated she got out of her vehicle to check on the damage and saw MV2 stopped further down the road. When she got back into her vehicle to meet with the operator of MV2, the vehicle was no longer there. No direction of travel of provided. Operator of MV1 and her daughter were not injured. There was minor damage to the front driver side bumper of MV1. I spoke with the JCC Facility Manager, Brian Morris (617-775-1719) who stated he will check the cameras to see if the cameras recorded the accident. I provided Mr. Morris with the time of the accident and my name and advised him to contact NPD if he has any information to provide. Prior to clearing, I canvassed the area for the vehicle with the description provided, yielding negative results.

On 05/10/19 I met with Luiz A. Lopes who stated his vehicle ( Mass Commercial Reg# T83695 White Toyota Tacoma P/ U) was struck while parked in the lot around 07:00. Lopes said he didn't call the Police right away because he wasn't sure his car was struck there. The truck sustained a cracked front bumper and the fog light was popped out. We went inside of National Lumber and spoke with a manager who said he would contact G.M. Greg Assarian and have him look at the security footage. On 05/14/19 at approximately 14:00 the Police Station received a call from Lopes who just got off the phone with Assarian who found some security footage of the incident. I then traveled over to National Lumber and met with Assarian. He showed me the video and it appears that a newer grey four door Chevrolet truck backed into Lopes truck around 06:58 of 05/07/19. The Massachusetts plate is unreadable but an older White heavy set male gets out of the truck and walks into the store. After parking the truck the male can be seen checking his own truck bumper for damage. The male is wearing a baseball hat and sun glasses and cannot be identified on the video. The male does however walk with a noticeable limp. Assarian will show his picture to his employees and contact me if the employees are able to identify the male driver. I called Lopes back and apprised him of my findings. ******************UPDATE**************************************** As of today 05/20/19 I have yet to hear back from anyone at National Lumber so I will end this investigation and label it a hit and run crash.

MV1 had just gotten out of her MV in the driveway of 111 Hammondswood Rd. While between the door and the MV the MV started rolling backwards down the sloped driveway. The operator grabbed a hold of the top of the door and was dragged backwards across the street into the yard of 104 Hammondswood Rd where it came to a stop. The operator initaially claimed minor injuries but signed a refusal. This accident was caused by operator error as it appears that the car was not completely in park. There was minor damage(tire track mark) to the lawn. I attempted to contact the homeowner with negative results. I took pictures of the damage and TOT IT.

Bike One was travelling Eastbound on Auburndale Ave when it appeared the operators front tire came in contact with a slightly raised manhole cover causing the operator to fall off his bike and impact the roadway. The cyclist was wearing a safety helmet but received a laceration around his eye and several lacerations to his arms and legs. He was transported to Beth Israel Boston. I spoke to the bicyclist who was unsure of how the crash occurred. I contacted his brother, Paul Colangelo (603 434-1251) who was advised of the incident. Photos were taken of the crash scene and emailed to the IT Bureau. I placed his bike into support services for safekeeping.

Operator of MV1 stated he was traveling southbound on Centre Street and came to a stop to allow three pedestrians and a dog to cross at the crosswalk at Allerton Rd. Operator of MV2 stated he observed MV1 stopping to allow people to cross and he came to a complete stop as well. Operator of MV3 stated she did not see MV2 stopped and did not see his brake lights on. MV1 said that when the pedestrians were about three quarters of the way crossed, he heard a loud crashed and saw MV2 swerve into the on coming traffic lane. MV2 made slight contact with the rear of MV1 causing scratches to the rear left bumper. MV2 said MV3 struck him from behind causing him to push forward. He swerved into the on coming lane to avoid hitting MV1. He continued to turn to the left in order to avoid the pedestrians that were still crossing. No contact was made with them. Newton Fire Engine 3 and Cataldo PB3 responded. MV2 was towed by David Donahue from Todys Towing. It sustained major rear end damage. The operator of MV2 was evaluated by Cataldo PB3 and signed a patient refusal. MV3 was towed by Bobby Donahue from Todys Towing. It sustained heavy front end damage, front air bag deployment and a small spill from the engine block. Operator of MV3 was evaluated and transported to NWH by PB3 for minor injuries. The three pedestrians witnessed the accident. All three stated that MV1 stopped to allow them to cross and they saw MV2 also come to a complete stop. MV3 struck MV2 from the rear causing him to swerve into the on coming lane to avoid hitting MV1 and them.

The tow truck operator ("Todisco Towing" truck with a ramp bed MA Repair plates 98BD) stated he was loading up MV#3 (An unregistered red colored BMW sedan being retrieved for auction bearing VIN WBABS33403PG88984) onto his inclined ramp bed when the tow hook on MV#3 broke causing MV#3 to roll backwards striking MV#2 which in turn pushed MV#2 into MV#1. Both MV#3 and MV#2 were unoccupied at the time of this accident and both vehicles were being towed for auction. There were no visible damages to the rear end of MV#3. There were minor damages to the rear left side of MV#2.

IT WAS LATER DETERMINED THAT JUST PRIOR TO VEHICLE #1 PASSING THE SCENE A POSTAL EMPLOYEE HAD JUST STARTED TO ATTEMPT TO TIE OFF THE WIRE TO A FENCE AT THE SAME TIME CAUSING THE ACCIDENT . VERIZON WAS CONTACTED EARLIER DUE TO A COMPLAINT OF A WIRE DOWN. PRIOR TO THE POSTAL WORKER ATTEMPTING TO PUT THE WIRE UP THERE WAS NO IMMEDIATE HAZARD IN THE ROAD. HOWEVER THE WIRE WAS BLOCKING THE SIDEWALK AND APPARENTLY THE POSTAL EMPLOYEE WAS CONCERNED THAT A PEDESTRIAN MIGHT TRIP ON IT.

There is no date or time associated with the video and I am unable to make out a license place number or identify the driver. I am unable to determine if this vehicle made contact with MV1.

On 06/30/2019 at 15:30 hours I was notified by Sgt Lee that a crash involving a motor vehicle and a bicyclist had occurred on Wiswall Rd at M Roadway. Upon my arrival at the crash scene I made the following observations, Wiswall Rd is a two lane non divided public way in the city of Newton that runs north to south from a dead end to Dedham St. The posted speed limit on Wiswall Rd is 25 MPH. M Roadway is a two lane non divided public way that runs west to east from Wiswall Rd to a dead end cul-de-sac. The weather conditions at the at of the crash were cloudy, and the roadway surface was wet from rain. I observed MA commercial registration M82794 a gray 2014 Honda Accord stopped in the north bound lane of Wiswall Rd at M Roadway. The Honda is registered to Newton Driving School. I observed two smudge marks approximately four inches in length on the center portion of the front and rear passenger side doors of the Honda. I also observed a 20 inch Kent bike that had be moved from it's final point of rest prior to my arrival. The bike had damage to it's front handle bars, front tire and it's two front forks. The bike's front tire had gray scuff marks on it. At the crash scene I spoke with the operator of the Honda, David Dombrovsky. Dombrovsky stated that he was driving north bound on Wiswall Rd near M Roadway approximately 20 to 25 MPH. Dombrovsky stated that he has had his learner's permit for six months and is a student at the Newton Driving School. Dombrovsky stated that he did not see the bicyclist but heard the impact of the bicyclist stricking the passenger side of the Honda. At the crash scene I spoke with driving school instructor, Igor Drizik. Drizik stated that the Honda was driving north on Wiswall Rd near M Roadway at approximately 20 to 25 MPH . Drizik stated that he did not see the bicyclist until approximately 1 to 2 seconds before impact on the driver's side of the Honda. Drizik stated the Honda is equipped with dual brakes and that he applied the brakes at the same time as Dombrovsky. Drizik stated that he and Dombrovsky got out of the Honda to render aid to the bicyclist. There were three other driving school students in the rear of the Honda at the time of the crash, neither of them stated that they witnessed the crash. None of the occupants of the Honda stated that they were injured and the vehicle did not require a tow. The bicyclist was identified as David Kerwin-Derrick. Kerwin-Derrick was transported via EMS to Boston Children's Hospital form treatment. At the hospital I spoke with one of Kerwin-Derrick's mother's, Patricia Kerwin-Derrick, who stated " We all know that David rode his bike into the car " and that her son had a broken left leg. Kerwin-Derrick is a student at the Memorial Spaulding School. Based upon my observations of the crash scene and statements made to me , I believe that Kerwin-Derrick's unintentionally rode his bike into the passenger side of the Honda. Photos were taken of the Honda, bike and crash scene and the disk was down loaded by the NPD's IT Bureau.

On July 3, 2019 at approximately 00:08 hours I responded to 25 Crosby Rd for a report of a hit and run that had just occured. Upon arrival, I spoke with Nicholas Giamalis who reported that he heard a bang and went outside and observed damage to MV1's left rear bumper. MV1 was parked facing southbound in front of 25 Crosby Rd. Nicholas said that a gray Chevy sedan driving northbound on Crosby Rd towards Commonwealth Ave. Officer Kayla Donahue canvased the area with negative results. Nicholas said that he was the last person to use the vehicle but the vehicle is owned by his father, Stephen Giamalis. On Tuesday July 9th 2019 at approximately 1409 hours I responded to a two car accident on Kenrick St and Waverley Avenue. At the time the weather was sunny and the road conditions were dry. Kenrick St and Waverley Ave are both public ways owned by the City of Newton. The operator of MV1 identified as Smreeti Khanal (S82370804) stated she was stopped at the red light on Kenrick Street and was waiting to turn left onto Waverley Ave when her vehicle MA REG 623KD7 was struck by a large box truck that was turning left onto Kenrick Street from Waverley Ave. MV1 sustained heavy front end damage and was towed from the scene by Todys. Two small children were in the back seats during the accident. No injuries were reported. The operator of MV2 identified as Keiven Vandon Stewart (TX 33693636) stated he had the green light and was turning left onto Kenrick St from Waverley Ave and the side of his truck made contact with MV1. MV2 stated MV1 was not on the right side of the road and did not leave enough space for the truck to make the turn. MV2 reported no injuries and sustained no damage. AMB1 responded and cleared with four patient refusals. MV2 was able to leave the scene.

On 07/18/2019 Gladys Bisono came into NPD Headquarters to report a past-hit and run. Bisono states that between 1500hrs on 7/13/2019 and 1100hrs on 7/14/2019 her leased vehicle (Vehicle 1) was parked and unoccupied on the eastbound side of Border St near 85 Border St. During this time frame an unknown vehicle (Vehicle 2) struck vehicle 1. On Tuesday, July 23, 2019, while assigned to Traffic unit N525, I responded to the intersection of Beacon Street and Washington Street, Newton for a motor vehicle accident involving a bicyclist. The weather at the time of the accident was overcast. The road surface was dry. Beacon Street and Washington Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Paige Mahoney (S85200426). Mahoney stated she was stopped in traffic at a red light on Beacon Street (W) at Washington Street in her 2018 black Volkswagon Tiguan (MA: 633MV6). Mahoney stated while her vehicle was at a complete stop, she felt and heard something hit the rear of her vehicle. Mahoney exited her vehicle and saw a bicyclist on the roadway. I observed minor damage to the rear middle bumper area of MV1. There was a small amount of blood present on the damaged area. I observed the roadway where the accident occurred. There was a small pool of blood in the left lane of Beacon Street (W) where the bicyclist fell to the roadway. The area of the roadway where the accident occurred is currently undergoing restoration and the asphalt has been graded down to it's sub base in preparation to be resurfaced. The current road surface condition is bumpy and uneven. The area where the roadway goes from a smooth surface to a bumpy surface is where the accident occurred. The injured bicyclist was identified as Timothy Johnson (S29270605). I spoke with him in the Newton Wellesley Hospital emergency room. Johnson stated he was operating his bicycle Westbound on Beacon Street on the right side of the left lane. Johnson stated he was coming to a stop in traffic at a red light when his wheel abruptly turned to the left and he began to fall over. Johnson stated his head hit the back of a vehicle stopped in front of him and he landed on his right side, with his bicycle landing on top of him. As a result of the accident, Johnson suffered an injury to the right side of his head and had scrapes on his right arm. Johnson stated he did not lose consciousness as a result of the accident and he appeared to be alert and orientated while I was speaking with him. Johnson was wearing a helmet at the time of the accident. There were fresh markings on the right side of the helmet with minor black paint transfer. These markings are consistent with Johnson's statement that he fell into the back of MV1 and hit his head. Johnson's bicycle was removed from the roadway prior to my arrival and was being stored in the driveway of 2044 Beacon Street by the resident there. I observed no obvious damage to Johnson's bicycle. After speaking with all parties involved and examining the roadway where the accident occurred, it appears that the condition of the road surface contributed to Johnson losing control of his bicycle and crashing into the rear bumper area of MV1 while it was stopped in traffic. Photos were taken of the road surface, John's bicycle, and MV2 and submitted to the IT Bureau.

Operator of MV3 stated he stopped at the stop sign on Lake Ave. then attempted to continue Northbound through the intersection. Operator of MV3 stated as he attempted to proceed through the intersection MV1 was flying down Beacon St. and collided with his vehicle. No injuries. Newton Fire and medics responded on scene and all parties involved signed patient refusals. Todys responded and towed MV1 and MV2. MV1 sustained substantial drivers side and left front end damage. MV2 sustained substantial left front end damage. MV3 sustained minor front end damage. It should be noted as I arrived on scene of the accident MV3 was in the middle of the intersection facing Northbound as indicated in the crash diagram. A towed motor vehicle form was filled out for both vehicles in regards and placed on file All debris from the accident was clear from the road including the bike lane.

Operator of vehicle 1 stated she was stopped at the stop sign on Crescent St at Auburn St when she saw 2 vehicles coming eastbound stopped to let her out. As she pulled out taking a left onto Auburn St she was hit on the drivers side by another vehicle which came from the opposite direction. The operator told everyone he was pulling over then quickly took off westbound on Auburn St towards Commonwealth Ave. The passenger of vehicle 1 stated that the driver of the vehicle was a black male in his early thirties driving a green possibly 2010-2012 Jeep Cherokee with a possible CT reg# of AP1042 or AP81042. He also stated that the driver looked like someone he tried to purchase a car off of in Waltham four months ago but had no further information. Three witnesses to the accident were in the two vehicles stopped to allow vehicle 1 to pull out of Crescent St. All three parties gave the same story that vehicle 1 was almost completely out of Crescent St when the green jeep coming westbound quickly accelerated and looked as if he was going to go around her but hit he drivers side then took off westbound. The vehicle description and possible plate were given out to surrounding towns and state police. Operator of vehicle 1 was transported by Cataldo to NWH for minor injuries. Vehicle 1 was towed from the scene by Todys.

When the traffic light turned green, MV#2 proceeded to move forward and struck MV#1 in its driver's side rear end and driver's side rear view mirror. The impact pushed MV#1 onto a dividing island to its right side. The operator of MV#1 complained of neck and back pain but refused medical attention. The witness stated he was stationary directly behind MV#2 and observed MV#2's rear end strike MV#1. The witness stated he did not believe the operator of MV#2 was aware he made contact with MV#1. The witness provided me with MV#2's registration of Indiana plates P687805. It should be noted that MV#2 is a large tractor trailer type vehicle. Newton Dispatch had advised me that State Trooper Capponi had located and stopped MV#2 on route MA Turnpike 90. The operator of MV#2 stated he was unaware he had made contact with any vehicle on the road. There were no visible damages to the entire right side of MV#2 or its trailer. There were no reported injuries to the operator of MV#2.

The operator of Newton Fire Ladder 3 (MA: MF453453), Michael Pohlman, stated while trying to exit Burr Park from the Waverly Street exit, the middle driver side tire area of Ladder 3 made contact with a fixed cement pole. The pole is located at the entrance/exit and has a chain attached to it used to restrict access to the area. I observed minor damage to the driver side middle tire area and no damage to the pole. No injuries were reported by the occupants of Ladder 3. Photos were taken and submitted to the IT Bureau. Ladder 3 stated they were at the Burr Park permorming an inspection.

On Wednesday, September 4th, 2019 while assigned to Traffic unit N526, I responded to 84 Paul Street, Newton for a report of a motor vehicle crash involving a City of Newton DPW vehicle. The weather at the time of the accident was clear and sunny. The road surface was dry. Paul Street is a public way maintained by the City of Newton. I met with the operator of MV1, Newton Associate Engineer John Daghlian. Daghlian stated he was operating Newton DPW Nissan Leaf (MA M99257) attempting to parallel park on Paul Street in an open space. Daghlian stated as he pulled forward into the space his front end made contact with the bumper of MV2 which was parked info of MV1 unoccupied. Both vehicles sustained very minor damage. No injuries were reported by the occupants of MV1. I spoke with the owner of MV2, Naomi Singer (2019 BMW 330XI, MA: 8SP372). She stated she was inside her dentist appointment when the accident occurred and was notified by Daghlian. Pictures of both vehicles were taken and submitted to the IT Bureau.

Upon discovering the damage, the owner of Vehicle 1 canvassed the area and located a surveillance camera at the Village Bank 332 Walnut St. The owner of Vehicle 1 was informed by Village Bank that they do not release surveillance video's to civilians and that he would need to report the incident to the police. It should be noted that this crash occurred on 09/11/2019 and was reported at the front desk of NPD Headquarters on 09/12/2019. I will be contacting Village Bank on 09/13/2019 regarding the surveillance video. Update: On 09/13/2019 I spoke with Lisa Bocabella of the Village Bank. Ms. Bocabella states that the exterior surveillance video at their Newtonville Branch only captures images of their front doorway and the adjacent sidewalk. Ms Bocabella states that their surveillance did not capture this crash. At this time the owner and operator of Vehicle 2 is unknown. The owner of Vehicle 1 was notified of these results.

Horvitz stated that Wegmans Supermarket may have video footage of the crash but would not release it to a civilian. I called Wegmans and spoke to Store Manager Chrissy Delmastro 617-762-2000. Delmastro stated she will notify her security team who will contact me back some time next week. I also contacted Dahn Pham 617-795-5999 of Chestnut Hill Square Security to inquire about their security footage. A message for Pham to contact me next week. On September 16, 2019 at approximately 17:50 hours I, Officer Guarino, responded to the area of 749 Beacon St. for a report of a motor vehicle crash involving a bicycle. Upon arrival with other units, we determined that MV 1 was parked in front of 749 Beacon St.The operator of MV1 opened the door to her vehicle to exit and her door struck a passing bicyclist. Traffic on Beacon St. westbound was stopped, the bicyclist fell off of the bicycle and sustained injuries and was transported to Newton-Wellesley Hospital for further evaluation.As the bicyclist was falling, he struck the rear of MV2, which did not appear to have any damage. The operator of MV1 said she did not see the bicyclist, as she went to exit her vehicle. Witness 1, who was stopped in traffic said that the bicyclist was traveling at a high speed and understood how MV1 did not see the bicycle coming. Officer D. Siegal transported the bicycle to the Newton Police Department and secured it in support services.

On Tuesday September 17th, 2019 at approximately 1411 hours I responded to the intersection of Fairfield Street and Bolton Road for a city vehicle accident. At the time, the weather conditions were clear and the road surface was dry. Fairfield Street and Bolton Road are both public ways in the City of Newton. Upon arrival I observed a City of Newton public works truck MA REG: M68227 front end crashed into a utility pole and small tree. The truck also had a trailer attached MA REG: M67846. The operator of the city vehicle identified as Rodolfo Marquez (S77096565) stated the vehicle was parked on the corner of Fairfield Street at Bolton Road with its hand break activated and a chock block under the rear right tire. Rodolfo stated he was attempting to load a bobcat onto the trailer when the truck must have released from the hand brake and crashed head on into a utility pole and small tree. The area in which the truck was parked on and point of impact are both on a downward hill. The impact from the truck into the utility pole caused a cable wire that was afixed to 36 Fairfield Street to be ripped from the house causing minor damage to the home. The city tree and utility pole also had minor damage. I spoke to the homeowner of 36 Fairfield Street identified as Nicholas Morgan. Nicholas was made aware of the damage to his house and damage to the front lawn from the vehicles tires. No injuries were reported and several photographs were taken of the vehicle and the damage to the house, lawn, city tree and utility pole. Verizon was notified to respond for a down cable wire. Todys responded and took posession of the vehicle.

THE OPERATOR HAD DASH CAM FOOTAGE OF THE ACCIDENT, AND IT SHOWED MV1 TRAVELING WESTBOUND ON BEACON ST APPROACHING THE VEHICLE PULLED TO THE SIDE OF THE ROAD. On 09/27/19 at 08:41 hours while assigned to Traffic Unit 523, I responded to a motor vehicle/bicyclist accident at 1330 Centre Street. The weather at the time was clear and sunny. The road surface was dry and Centre Street is a public way maintained by the City of Newton. Traffic conditions were at a heavy volume and Centre Street was congested at the time. There is no dedicated bicycle lane in this area due to the road being too narrow. I spoke to the bicyclist involved (Unit 2), Michael Hnatko (S10432772). Mr. Hnatko stated he was traveling northbound on Centre Street on the right side of the road on his 20 inch gray Trek bicycle. He further says that he was peddling very fast at the time and must have not been paying attention to MV1 turning right in to parking lot of 1330 Centre Street as he was approaching from behind. As MV1 was turning right, he then struck the passenger side rear quarter panel of the vehicle and fell forward off of his bicycle. He sustained minor injuries such as scrapes to his knuckles on both hands and a small cut to the bridge of his nose. He signed a patient refusal with Cataldo Ambulance. It appeared that the front alignment was now off on his Trek bicycle. I then spoke to the operator of MV1, Lonnie McGinnis (S66044083). He states that he was inching along in heavy traffic waiting to turn right in to the lot of 1330 Centre Street with his right turn directional signal on. As he was turning right, the bicyclist struck his vehicle on his passenger side rear quarter panel area. McGinnis thought it was clear to turn right at the time and never saw the bicyclist approaching from behind. No injuries reported by McGinnis and I observed minor dents and scratches to the passenger rear quarter panel area. Digital photos taken by Ofc. Gaudet.

Operator of motor vehicle 1 states he came to a stop on Grove Street headed north at the stop sign at the intersection of Woodland Rd. Vehicles on Woodland Rd. stopped to let him go. As motor vehicle 1 proceeded into the intersection a bicycle came out of no where and the operator of motor vehicle 1 struck the bicycle at a low speed with the front of motor vehicle 1. The cyclist states he was traveling east on Woodland Rd. when he proceeded into the intersection he was struck on his right side by motor vehicle 1. The crash was at a low speed and the bicycle tipped over to the left side, causing an injury to his left arm. The cyclist states it was hard to see due to the rain. When the cyclist was asked if he had stopped at the intersection he stated he was not sure if he stopped or just slowed down. When asked if he came to a complete stop and placed a foot onto the ground, the cyclist stated no. The cyclist stated normally when he stops he just balances in place and doe not take his foot off the pedals. When I asked the cyclist if he passed other vehicles on the right to get up to the intersection he stated yes, he normally does so. It should be noted there is no dedicated bike lane and both streets are narrow at this intersection. MED2 transported the cyclist to NWH for his arm injury. I took pictures of the roadway as well and the bicycle and motor vehicle 1. N497 transported the bicycle to property and evidence for safe keeping, N497 submitted memory chip with photos to IT.

Lauren state that she was involved in this accident and states that vehicle #2 continued through the intersection on Chestnut St. Lauren got out of her vehicle and saw she had damage to the right rear of her car. On Wednesday October 23rd, 2019 at approximately 1306 hours I responded to 40 Beaconwood Road for a motor vehicle accident involving a city vehicle. At the time of the accident the weather conditions were sunny and the road conditions were dry. Beaconwood Road is a public way owned by the City of Newton. Upon arrival I spoke with the operator of the city vehicle (MV1) identified as Norayr Sargsyan ( S34138376). Norayr stated that he was traveling southbound on Beaconwood Rd operating a yellow 2009 Ford drwsup truck (MA COM REG; M2939A) and as he passed a landscaping truck that was parked, the rear drivers side corner of the city truck made contact with the rear drivers side of the landscaping trailer. Norayr stated there was a car parked facing the wrong direction directly across from the trailer making it hard for him to pass in between both vehicles. No damage to the city truck was visible, minor damage was present on the trailer. Photographs were taken of the damage and submitted to the IT bureau. No injuries were reported. I spoke with the operator of the landscaping truck (MV2) identified as Jose Nunez (S24348117). Jose stated MV2, a red 2008 Ford drwsup dump truck was unoccupied and parked with a red 2012 Brav utility trailer (MV3) attached to it. Jose reiterated the same sequence of events as Norayr. Both vehicles were able to leave the scene without further incident and I advised both parties a report would be submitted.

The operator of vehicle 1 stated that she was traveling North on Winchester Street when vehicle 2 came out from her right side at a high rate of speed and cut her path of travel off. It happened too quickly for her to stop and she crashed head on into the driver side front corner of vehicle 2. She stated she was on the left side of the lane but was traveling with the normal flow of traffic. This caused minor damage to vehicle 2 which was able to be driven away, vehicle 1 sustained major damage to the front bumper and hood and needed to be towed. The operator of vehicle 2 stated he was turning left onto Winchester Street after he had stopped at the stop sign. He stated traffic had stopped to let him turn, he proceeded to do so and vehicle 1 came from the far side of the lane, going around stopped traffic, and crashed into his car.

Narrative:
On Thursday, October 24, 2019, while assigned to Traffic unit N525, I responded to the intersection of Commonwealth Avenue and Cedar Street, Newton for a report of a motor vehicle crash involving a bicyclist. The weather at the time of the crash was clear and sunny. The road surface was dry. Commonwealth Avenue and Cedar Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Samuel Ntonme (S52001288). Samuel Ntonme stated he was operating his 2009 Nissan Murano (MA: 192VH6) Northbound on Cedar Street at Commonwealth Avenue. Ntonme stated he crossed Commonwealth Avenue and entered the carriage road area of Commonwealth Avenue to continue onto Cedar Street when a bicyclist crashed into the front driver side bumper/fender area of his vehicle. Ntonme stated there was a delivery truck partially blocking the intersection of Cedar Street (S ) at the Commonwealth Avenue carriage road. Ntonme stated he did not see the bicyclist until it crashed into his vehicle and stated the vehicle was traveling Eastbound on the roadway in the Westbound lane. I observed minor damage to the front driver side bumper/fender area of MV1. Ntonme reported no injuries. The operator of the bicycle, Ezra Robison (S83132607), was transported to Newton Wellesley Hospital with a leg injury prior to my arrival. I observed damage to the front and rear wheels on the Robison's bicycle on scene. Robison's bicycle was transported to the Newton Police Station for safe keeping. I went to Newton Wellesley Hospital to obtain a statement from Robison but he was discharged prior to my arrival. I spoke with Robison at his residence later in the day. Robison stated he was operating his bicycle on the Commonwealth Avenue carriage road (E) towards Cedar Street. Robison stated there was a delivery truck stationary on Cedar Street partially in the carriage lane. Robison stated he went around the delivery truck and didn't see MV1 until impact. Robison stated he crashed into the front driver side wheel area. Robison stated the front wheel of his bicycle went under the front driver tire of MV1 and he fell to the roadway with his bicycle. Robison stated he has minor injuries as a result of the crash. Pictures were taken of the crash scene and submitted to the IT Bureau.

The operator of MV1 (MA: 249AB2) stated he was operating his vehicle Westbound through the parking lot behind 1321 Washington Street, Newton. The operator stated he was attempting to pull over to the right side of the roadway to allow a vehicle traveling Eastbound through the parking lot by him. In doing so, MV1's passenger side door area made contact with the front crash guard of Newton Police cruiser N506 ( MA MVN: MP506B). I observed minor damage to the passenger side door area of MV1 and minor damage to the front crash guard of MV2. No injuries reported. Photos were taken and submitted to the IT Bureau.

On October 25, 2019 at approx. 1847 hours, I responded to 574 Auburn St for a report of a MVA. Upon arrival, I spoke with MV1 operator who stated he was travelling Westbound on Auburn St when his vehicles front axle suddenly broke causing the vehicle's front right wheel to turn outboard thus causing the MV to drive right. MV1 then collided with MV2 which was parked outside of 574 Auburn St. MV1 was towed by AAA. No injuries reported. Around 2130 hours, Mike Woods called the police station to receive information regarding the crash and MV2. He was updated and informed me that he would also be calling AAA for his vehicle.

Vehicle #1 was parked facing south in front of 7 Spaulidng La. Vehicle #1 had a trailer with an excavator attached to it. Vehicle #1 was unoccupied but had one occupant in the excavator on the trailer. The operator/owner of vehicle #1 was moving the excavator off the trailer when the trailer and truck started to roll southbound down the hill on Spaulding La. The operator of vehicle #1 jumped out of the excavator onto the roadway. Vehicle #1 along with its trailer rolled down and veered off the road and over the curb. The rear driver side end of the of vehicle #1 collided with the rear end of vehicle #2 bringing the trailer and truck to a stop. The rear end of vehicle #2 had moderate damage. The rear end of vehicle #1 had minor damage. Vehicle #2 was parked facing southbound just south of the driveway of 7 Spaulding La. Vehicle #2 was parked on the side of the road on Spaulding La. and was crashed into by the unoccupied truck and trailer vehicle #1. The occupant of vehicle #2 was not injured. The occupant of vehicle #1 was transported to Beth Israel Boston by Newton Amb. 1. There were no tows needed. Reference incident report #19046951 for further details on this crash.

On Thursday November 14th, 2019 at approximately 1005 hours while working in unit N526, I responded to the area of St. James Street and Washington Street for a motor vehicle accident involving a City of Newton DPW truck. Both St. James Street and Washington Street are owned and maintained by the City of Newton. At the time of the accident the weather conditions were cloudy and the road surface was dry. The operator of MV1 identified as Nicholas Shabanian (S36948722) stated he was operating a City of Newton dump truck MA REG: M2533A and was travelling southbound on St. James Street merging onto Washington Street. Nicholas stated MV2 made a sudden stop at the red light on Washington Street, and he was unable to stop the truck in time and made contact with the rear end of MV2. Nicholas was not injured and MV1 sustained no visible damage. The operator of MV2 identified as Sarah Hodin (S29245836) stated she operating a 2013 Toyota Camry MA REG: 1ADB47 and was stopped at the red light on St. James Street and Washington Street. Sarah stated while she was stopped MV1 struck her vehicle from behind. Sarah was evaluated by AMB2 and signed a patient refusal. MV2 sustained minor damage to the rear end and bumper. Pictures of the accident were taken and submitted to the IT Bureau.

OWNER GAVE ME A MASS REG #43559 ( PLATE DOES NOT COME BACK AS BELONGING TO ANY KNOWN VEHICLE ). OWNER STATES HER SON WAS ALSO IN THE VEHICLE BUT NEITHER SHE NOR HER SON WERE HURT. VEHICLE #1 HAS MINOR LEFT SIDE DAMAGE. OWNER ADVISED TO CONTACT HER INSURANCE COMPANY. Traffic Bureau Update (Officer Gaudet): On Tuesday, December 10, 2019, I made contact with the registered owner of MV2, Christopher Mearls (S44004540). Mearls is the owner of a 2013 Nissan Altima (MA: 43559). Mearls stated he is in the military and was on base in Springfield, Massachusetts over the weekend with his vehicle. Mearls stated no one else drives his vehicle.

The operator of MV#1, identified as Frank Vanmierlo states that he parked his MV in the Marriott parking area at approx. 7:45AM on 12/9/19. When he returned to his unoccupied MV he found MV#2 making contact with the rear of his MV and his MV was now moved slightly from its original parking spot. MV#2 was unoccupied when Mr. Vanmierlo returned to his MV and we could not locate the registered owner on the Marriott property. I then spoke with the operator of MV#2 by telephone, identified as Jonathan Lang. Mr. Lang stated that he parked MV#2 in the Marriott parking area at approx. 7:45AM and was not involved in a crash with any other MV while in the parking area. Mr. Lang states that there was another MV parked and unoccupied in the parking space in front of him when he turned left into his parking spot. Marriott security states that there are no cameras video taping the exterior of the Marriott including the parking area. Interior videotape provided by the Marriott confirms that Mr. Lang entered the front lobby before Mr. Vanmierlo. Upon further investigation it was determined that an unknown person had informed Mr. Vanmierlo that a Red Acura had struck his MV while parked in the parking area. Mr. Vanmierlo also confirmed that there was new damage to the front of his MV. No other information available at this time. It would appear at this time that unknown MV#3 backed into the front of MV#1, pushing MV#1 into the front of MV#2.

On Monday, December 9, 2019, while assigned to Traffic unit N525, I responded to the intersection of Lincoln Street and Walnut Street, Newton for a report of a motor vehicle crash involving a pedestrian. The weather at the time of the crash was rain. The road surface was wet. Lincoln Street and Walnut Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jill Edelson (S99137352). Edelson stated she was stopped in traffic while operating her 2012 Toyota Camry (MA: 81ZH83) Northbound on Walnut Street at Lincoln Street at a red light. Edelson stated the light turned green and she had a green left turn only arrow. Edelson stated she turned left into Lincoln Street from Walnut Street when the front passenger side of her vehicle crashed into a pedestrian in the roadway. Edelson stated she did not see the pedestrian prior to the impact. I observed damage to the lower portion of the passenger side windshield. Edelson reported no injuries. I spoke with the injured pedestrian, Lawrence Murren, at Brigham and Women's Hospital. Murren stated he had entered the crosswalk on Lincoln Street at Walnut Street and does not remember if he had the pedestrian signal. Murren believes MV1 did have the green light. Murren stated after he entered the crosswalk he looked back towards the "59 Bus". Murren stated he then looked forward and saw MV1 was in front of him. Murren stated he was unable to avoid the vehicle. Murren stated he has an injury to his hip and had a head laceration. I spoke with a witness to the accident, Edward Austin. Austin stated he was in the vehicle behind MV1 at the traffic light on Walnut street. Austin stated the traffic light turned green and MV1 had a green left turn arrow. Austin stated MV1 started to go and make their left turn. Austin stated he observed an older gentleman starting to walk out into the street. Austin stated the pedestrian had his head down and had an umbrella due to the rain. Austin stated the pedestrian also had a do no walk sign. Austin stated MV1 hit the crossing pedestrian in the crosswalk. Austin stated he does not believe either saw each other. Photos were taken of the area and submitted to the IT Bureau.
