ON 2-1-19 AT APPROX. 1340HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT HIT AND RUN. UPON ARRIVAL AT 72 HIGHLAND ST. I SPOKE TO THE OWNER O VEHICLE #!. OWNER STATES HE WAS INFORMED BY A COWORKER ( NO LONGER ON SCENE ) THAT A VAN ( MA REG: T65665 ) SIDESWIPED HIS PARKED MOTOR VEHICLE CAUSING THE LEFT REAR VIEW MIRROR TO BE DAMAGED. OWNER STATES HIS VEHICLE WAS PARKED ALL DAY BECAUSE HE IS DOING WORK AT THAT LOCATION. COWORKER GOT THE PLATE OF THE VEHICLE THAT HIT HIS VEHICLE. OWNER REPORTS NO INJURY AND WAS ADVISED O CONTACT HIS INSURANCE COMPANY. ATTEMPTS TO MAKE CONTACT WITH THE OWNER OF VEHICLE #2 WAS UNSUCCESSFUL AT THIS TIME. FOUND A SECOND NUMBER ( 860-301-1336 ) FOR THE METROWEST PACKAGING COMPANY AND LEFT ANOTHER VOICE MAIL.WILL TRY TO MAKE CONTACT AGAIN BEFORE PURSUING OTHER OPTIONS. Traffic Bureau Update (Officer Gaudet): On Monday, February 4, 2019 I made contact with the President of Metrowest Packaging, Brian Hersh. Hersh stated his company does fulfillment deliveries for Amazon. Hersh was advised that a vehicle registered to his company was allegedly involved in a hit and run accident. Hersh was provided information on the vehicle involved. Hersh stated an employee named Alexis was operating MA CON:T65665 on the date of the accident and provided a phone number for this party. On Tuesday, February 5, 2019 I spoke with Alexis Laporte-Soto (S84358654). Laporte-Soto stated he was operating a 2017 Mercedes Sprint van (MA CON:T65665) in Newton for Metrowest Packaging on the date of the accident. I explained to Laporte-Soto the vehicle he was operating was alleged to have been involved in a hit and run accident on Highland Street in Newton. Highland Street is a public way maintained by the City of Newton. Laporte-Soto stated the vehicle he was operating hit the driver door mirror of a parked card on Highland Street. Laporte-Soto reported the accident as mirror (MV2 passenger side) to mirror (MV1 driver side). Laporte-Soto stated after his passenger side door mirror made contact with MV1's driver side door mirror, he stopped his vehicle 15 feet down the road. Laporte-Soto stated he walked up to the car he hit and saw the mirror was bent back but there was no damage. Laporte-Soto stated he waited near the car for 10 minutes and the owner never showed up. Laporte-Soto also stated a "random guy" took a picture of his license plate. Laporte-Soto stated he said "hi" to this party but the party ignored him and left the area. I asked Laporte-Soto why he did not call Newton Police to assist him in finding the owner of the vehicle he hit. Laporte-Soto stated he didn't know. I also advised Laporte-Soto that the name of the owner of the vehicle he hit was printed on the driver side door next. The owner's phone number and email address were also printed on the driver door. Laporte-Soto stated he didn't see that. I spoke with the owner of MV1, Jason Correria. Correria stated a co-worker observed the entire accident and observed MV2's passenger side mirror hit MV1's driver side mirror. Correria stated his co-worker then observed MV2 travel down Highland Street with out stopping and pull into the Santander parking lot located at 1314 Washington Street. Correria stated his co-worker saw the operator of MV2 exit his vehicle, check his passenger side door mirror for damage, and leave the area. Laporte-Soto will be mailled Massachusetts Uniform Citation T1443121 for Chapter 90, Section 24 (Leaving the scene of property damage).

The operator of MV1 stated she was parked SB in front of 175 Temple St. from approximately 1000 hrs to 1430 hrs, and she came out to a note left on her vehicle. The note stated "I saw someone back into your car, I got their plate, MA Reg TC58LC, Black Suburban". and there was minor damage to the drivers side doors. MA Reg TC58LC came back to a HUTTER, Carol of 67 Randlett Pk, Newton. I responded to that address and attempted to raise HUTTER with no answer at the door. I then called the phone number listed from a past incident with the Newton Police and HUTTER answered the phone. She stated she was at the Temple attempting to parallel park when she thought she may have hit another vehicle. She stated she got out to look at the vehicle and didn't see any damage, so she took a photo of the license plate and would try to report it once she got home. MA Reg TC58LC had several scratches on the bumper of the vehicle.

OPERATOR OF MV 1 STATED HE WAS TRAVELING SOUTH ON CRAFTS ST AND AS HE WAS PASSING THE ARS RESTORATION SPECIALISTS PARKING LOT WHICH WAS ON HIS RIGHT, HE WAS STRUCK BY MV 2 WHO WAS BACKING OUT OF THE PARKING LOT AND INTO TRAFFIC ON CRAFTS ST. MV 2 WAS PLOWING THE SNOW IN THE PARKING LOT AND AS HE WAS BACKING UP, HIS PASSENGER SIDE REAR BUMPER STRUCK MV 1 ON THE DRIVERS SIDE. BOTH VEHICLES HAD MODERATE DAMAGE AND NO INJURIES WERE REPORTED. I TOOK SEVERAL PHOTOS OF THE ACCIDENT AND PUT THEM IN IT'S BOX TO BE DOWNLOADED.

The owner of MV#1 stated her vehicle was parked in the rear parking lot (Located on Rice St) of 1066 Centre St and was informed by 2 witnesses that stated her vehicle was struck by MV#2. I spoke to the 2 witnesses and they stated they were on the second floor of 1066 Centre St in suite #212 and had a bird's eye view of the rear parking lot on Rice St. The witnesses stated that MV#1 was parked while MV#2 was parked at an angle at a very close proximity to MV#1. The witnesses stated the operator of MV#2 was cleaning the snow off her vehicle, got into her vehicle then backed out of her parking space striking MV#1. The witnesses took a digital photograph of MV#2's registration (MA REG 763YC9). They described the operator of MV#2 as an Asian female in her 40's, 5'1" in height, operating a black colored "Buick" S.U.V type vehicle. MV#1 sustained minor damages to its front passenger door and bumper. A query of MA REG 763YC9 showed that it was registered to an Asian female to a black colored "Buick Rendezvous". The owner of MV#1 stated the owner of MV#2 also has a business in the same building at 630 Commonwealth Ave (Move2Boston Group). I spoke to the registered owner of MV#2 via telephone and she stated she was parked in the rear parking lot on Rice St but denied making contact with any vehicles. I could not observe MV#2 as it was not at the scene. At 14:40 I returned to 1066 Centre St and was able to observe MV#2. MV#2 displayed minor scrapes to its front driver's side bumper which was consistent relative to where both vehicles were parked. Based on the witnesses statements I will document this accident and provide this report number to the vehicle owners.

MV1 had just picked up a student at 56 Crescent Ave(at the intersection of Throwbridge). He made the pick up just west of the stop sign. He turned off his illuminated bus stop sign, paused and then began to pull into traffic. MV2 was stopped at the stop sign on Trowbridge st at Crescent St waiting for the bus to turn off the stop sign. When the bus turned it off, it appeared to him that the bus was not pulling into traffic. MV2 began to turn left "very slowly". As he was driving next to the van, the van pulled into traffic and the 2 MVs touched mirrors. There was no damage to either MV. A BLS ambulance was called to the scene because there were three students on board. All parents showed up and signed refusals for their children. Pictures were taken of the two mirrors that had no damage. The following parents showed up on scene to sign refusals for their children: Gareth Gorman for Sophia 617-515-1576 Lori for Harry Melchionno 617-527-7751 Malagros Falu for Nathalie Cedno The students belong to the Oak Hill Middle school and they were notified.

On 03/19/19 at 14:57 hours, I responded to 220 Dedham Street for a past motor vehicle hit and run accident involving a parked car that occurred at 12:26 hours on this date. Dedham Street is a public way maintained by the City of Newton. The owner of M/V # 1 states her vehicle was legally parked in front of 220 Dedham Street facing northbound when she came out and found a note from a witness saying it was struck by a gray Honda Accord bearing MA Reg. # 8512HV. It appears that the operator of M/V # 2 side swiped M/V # 1 as it was passing by on Dedham Street. I observed marks to the driver's side rear door area and the side view mirror was broken. On 03/20/19 at 08:47 hours, I spoke to the witness, Kawtar Bhih, on the phone and she stated that she was parked on the opposite side of where the accident took place. Bhih observed a gray Honda Accord with an elderly female operating M/V # 2 on Dedham Street almost side swiped another parked car before side swiping M/V # 1. She says that the vehicle slowed down after and then continued to leave the scene of the accident. She then took a photo of the above listed registration plate. I attempted to contact the owner of M/V # 2 (Jali Medical Inc Waltham, MA) and the receptionist was not able to provide any information at the time. I was told to call back the next day. I gave the vehicle and registration info to the person inquiring if they could find a possible driver and call back. While working on this report, I observed that the owner of M/V # 2 was involved in a past accident in our system and the operator at the time had an address of 66 Country Club Rd. Newton. On 03/20/19 at approx. 09:30 hours, I went over to this address and observed M/V # 2 in the driveway with an operator inside. After inquiring about the accident, she stated she was driving down Dedham Street when she heard a noise coming from her passenger side view mirror as she had struck something. She then left the scene not thinking it was serious. I observed fresh damage to her passenger side mirror along with other marks along the doors which appeared to be older. Operator was issued in hand MA Uniform Citation # T1441710 for Ch. 90/S24 Leaving the scene of property damage.

VEHICLE #1 STOPPED BEHIND HER AND AS HE GOT OUT OF HIS VEHICLE, VEHICLE #2 LEFT THE AREA. VEHICLE #1 WAS ABLE TO TAKE A PICTURE OF VEHICLE #2 PLATE ( MA REG: 76439 ) AS THEY LEFT THE AREA. ATTEMPTS TO CONTACT VEHICLE #2 WERE UNSUCCESSFUL. WILL TRY AGAIN ON NEXT SHIFT. VEHICLE #1 ADVISED TO CONTACT HIS INSURANCE COMPANY. ON 4-3-19 AT APPROX 1400HRS. I WAS ABLE TO MAKE CONTACT WITH THE OPERATOR OF VEHICLE #2 ( MARJORIE TEMPLE ). OPERATOR EXPLAINED TO ME THAT SHE WAS TRYING TO GET IN THE RIGHT LANE FOR THE WOODLAND PARKING GARAGE AND DID NOT SEE VEHICLE #1 BECAUSE HE WAS IN HER BLIND SPOT. SHE STATED SHE PULLED OVER AND CHECKED HER VEHICLE AND SAW NO DAMAGE. OPERATOR STATES AS AN ELDERLY FEMALE WHEN A YOUNG MALE, 6'5" TALL GETS OUT OF HIS VEHICLE ANGRY I WASN'T GOING TO CONFRONT HIM. PARTY WAS ADVISED TO USE GREATER CAUTION WHILE DRIVING IN THE FUTURE. PARTY ALSO ADVISED TO CONTACT HER INSURANCE COMPANY.

MV1 was backing out of her parking spot in the parking lot at 916 Walnut St when she stopped to allow a pedestrian to cross in front of her vehicle. While she was waiting for the pedestrian to cross safely a vehicle backing out of the parking space across from hers backed into her vehicle and left the scene. Operator 1 was able to obtain a photo of the license plate. I then was able to contact operator 2 on the phone who stated she was at 916 Walnut St and "tapped" a vehicle when leaving and then left the scene. No injuries and both vehicle driven from the scene. MV1 sustained minor damage to the passenger side. Operator 2 states there is no damage to MV2. Operator 2 issued and mailed MA Uniform Citation T1442192 for leaving the scene property damage.

Patrice Lys was operating vehicle #2 on Brandies Rd. Lys states that vehicle #1 was about to back out of the driveway so he stopped. Lys state that the vehicle did not back out so he started to drive again, at that point vehicle #1 backed out of the driveway and struck his vehicle on the left rear. Azad Chitchian was operating vehicle #1. Azad states that she saw vehicle #2 stop to let her out of her driveway, she then went to back out of her driveway and vehicle #2 struck her. Aghavni Gevorgyan was witness. Aghavni confirms Azad's version of events. Aghavni and Azad are known to each other. Azad was transported to NWH. I took pictures, no traffic officers working.

On Tuesday May 7th, 2019 at approximately 13:15 hours I responded to the area of 30 Langley Road for an auto vs. pedestrian accident. At the time of the accident the weather was sunny and the road conditions were dry. Langley Road is a public way maintained by the City of Newton. Upon arrival I first spoke to the pedestrian who was inside the ambulance getting medical attention. The pedestrian identified as Donna Kalikow (S24151638) stated she was standing in an open parking space for a few seconds, attempting to walk across the street to enter J.P. Licks. Donna stated she was standing in the marked parking space waiting for traffic to stop to safely cross the street. Donna states the cars stopped to let her cross and she proceeded into the roadway. She states MV1 was attempting to back into the parking space that she was standing in and struck her right side and knocked her to the ground. Donna states the operator of MV1 stopped the vehicle when he realized he had struck her and got out to contact 911. Donna appeared to have minor injuries and complained of head pain. Medics transported Donna to NWH to be further evaluated. The operator of MV1 identified as Christopher Moos (S24090966) stated he was attempting to parallel park into an empty parking space and never saw the pedestrian standing in the space. Christopher stated as he was backing up into the space he was notified by a bystander that he just struck a pedestrian. Christopher stated he immediately stopped the vehicle and got out to assist the pedestrian and contact 911. Christopher stated he was not travelling no more than 5mph at the time of the accident. MV1 did not have any damage to the vehicle and was able to leave the scene. Christopher reported no injuries. Several photographs were taken of the scene and submitted to the IT Bureau. I mailed the operator a Massachussetts Uniform Citation for a City of Newton Ordinance violation for Section 19-75 Failure to use care when backing.

Tipruthai Thangthong stated that on 05/21/2019 at approximately 16:36 hours she was pulling into the driveway of her sisters home at 53 Rangeley Rd while operating her vehicle MA reg 2LVL61 a 2018 Honda Accord. Thangthong stated that her three old son Mighty Mettanant was a passenger in her vehicle. Thangthong stated that she was driving approximately 5 MPH as she drove into the driveway. The driveway runs east to west and Thangthong's vehicle was heading west into the driveway as the sun was in the Western sky creating a glare. Thangthong stated that she did no see anything in the driveway prior to her pulling into it. Thangthong stated that she felt the front of her vehicle strike what she described as " something " and stopped her vehicle. Thangthong stated that she put her vehicle in reverse and then into park and exited her vehicle to see what she had struck. Thangthong stated that she saw her two nieces, Kalla Mongkhonlertsirkul age three and Kelley Mongkhonlertsirkul age seven laying in the driveway in front of her vehicle. Thangthong stated that she then realized that her vehicle had stuck her two nieces as they were in the driveway. Both of the Mongkhonlertsirkul sisters were conscious and alert and were able to walk to an ambulance for transport to Newton Wellesley Hospital for treatment. Thangthong stated that she and her son were not injured in the crash. I observed no damage to Thangthong's vehicle. I responded to the Newton Wellesley Hospital and spoke with the Mongkhonlertsirkul sisters and their mother Piyada Kellum. Kellum stated that her oldest daughter Kelly is a student at the Franklin Elementary School . Kellum stated that her daughters told her that they were playing in their driveway when their aunt's vehicle entered the driveway and did not stop. I spoke with both of the Mongkhonlertsirkul sisters who told me that they tried to get out of the way of their aunt's vehicle but could not in time. The Mongkhonlertsirkul sisters stated that they were both struck in the back and fell to the ground between the two front tires of their aunt's vehicle. I spoke with hospital staff who stated that there did not appear to be any broken bones or other internal injures. I took photos of the crash area and forwarded the disk to the NPD's IT bureau for downloading.

Operator of vehicle one, Benjamin Beaver stated that while driving south bound near 321 Walnut St his vehicle ( MA reg 551DK8 2014 Lexus ) made contact with a pedestrian in a marked crosswalk. Walnut St is a public way in the city of Newton. Beaver stated that he did not see the pedestrian prior to the crash and that his vehicle was travailing approximately 5 to 10 MPH. Beaver stated that the pedestrian was knocked to the ground but got up and walked around. The pedestrian, NNHS student Benjamin Donovan stated that he was crossing east to west in the marked crosswalk in front of 321 Walnut St. Donovan stated that he saw Beaver's vehicle approaching him at approximately 5 to 10 MPH. Believing that Beaver was stopping to let him cross , Donovan stated that he held up his left hand to signal to Beaver that he was going to cross. Donovan stated that Beaver's vehicle struck him on his left side and knocked him to the ground. Donovan stated that he had minor scrapes on his right side as a result of falling to the ground. Donovan stated that he did not lose consciousness and was transported via EMS to Newton Wellesley Hospital for treatment. I took photos of Beaver's vehicle and forward the disk to NPD IT Bureau. I issued Beaver MA Citation T 1269995 and cited him for a violation of MGL Ch 89 C 11, failure to yield to a pedestrian in a crosswalk. On 06/12/2019 I was forwarded an E-mail from Capt Anastasia of a video of the crash that was taken from a camera that was mounted on the roof of 321 Walnut St. The video shows Donovan walking into the marked crosswalk from the east side of Walnut St and hold up his left had to Beaver's on oncoming vehicle. Beaver vehicle appears to be travailing faster than the 5 to 10 MPH that Beaver believed that he was travailing. The video shows that Beaver did not brake until Donovan was struck and landed on his vehicles hood. Donovan was thrown approximately 10 feet from Beaver's vehicle and landed on his right side on the roadway. The video shows Beaver stopping and pulling to the side of the roadway after the crash.

On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

There was airbag deployment and heavy damage to veh #1. Operator of veh #1 had some cuts and scrapes to legs but was checked out by the Medics and signed a patient refusal. The vehicle was towed by Tody's towing and the operator was issued Mass Citation #T1445105 in hand for CH89/ Sec4a Marked Lanes violation (traveling outside of breakdown lane) and Ch 90/ Sec 18 Speeding (based upon the significant amount of damage to the vehicle and how far the debris was found from the crash site) . The operator was picked up on scene by his daughter. Traffic Officer McSweeney arrived and took pictures of the damage to the city tree.

On 08/06/19 at 12:34 hours, I responded to Industrial Place by The Lakeshore Learning Store at the request of Newton Police Officer Justin March who witnessed an accident in the parking lot of 300 Needham Street. Officer March was working a road construction detail at the time in the City of Newton. He states that at approximately 12:20 hours he observed MV1 exiting the parking lot of 300 Needham Street( a drive way to a building that the public has access to). There is an island in the middle that separates the entrance and exit of the lot as it goes out to Needham Street. As MV1 was exiting, he drove up on the island and pushed two large stone builders out of place. There was also damage to numerous different flowers planted in the ground. MV1 then exited the lot and went eastbound on Needham Street. Officer March then followed in his vehicle and kept beeping his horn to alert the driver to stop. The operator turned on to Industrial Place and pulled over. Ofc. March attempted to speak to the operator, Gelvin Perez-Ramirez but there was a language barrier. Next, Ofc. Calderon responded and assisted with Spanish translation. He asked the operator what occurred and he stated that he never saw the island. Due to the fact that the operator left the scene of the accident and never reported it, he was issued MA Uniform Citation # T1445351 for Ch. 90/24/C. MV1 sustained damage to the two exhaust pipes on the passenger side of the truck. There is also possible undercarriage damage. Ofc. M. Wade took photos of the scene.

After parking, she was sitting in her motor vehicle when she heard a bump to her driver's side rear end from MV2 backing up from a parking spot across from her. She says the operator of MV2 turned and looked at her and kept proceeding (NB) on Charlemont towards Needham Street. She then exited her vehicle to try and get the operator's attention to no avail. She took a photo of MV2 which was a black Toyota Highlander bearing MA reg.# R85045. She described the operator as a white female in her forties with possibly dark curly hair. I observed minor paint damage and scratches to the driver's side rear quarter panel area. The operator is unsure if that damage is from the accident. The owner of MV2, Steven Masterson has a residential address of 40 Wild Acres Rd in North Attleboro, MA and a mailing address of 34 Central Street Foxborough, MA. A telephone search revealed a phone number of (269)857-7814 for him out of Aiken, SC and the Foxborough address but the number is not in service. UPDATE: I was provided a phone number for Masterson from N. Attleboro PD. On 08/16/19 at 12:45 hours, I spoke to Sheila Masterson who stated she was driving MV2 at the time of the reported accident. She stated that she was unaware that she struck MV1 and saw the other operator looking at her but thought nothing of it. Due to the accident being very minor or possibly no damage at all, no further action will be taken. Masterson was provided with the other operator's information.

I left a VM with the owner of MV2 requesting a call back. MV1 sustained minor passenger side damage. It is unknown what damage was caused to MV2 and whether it would be to the truck or the trailer. Traffic Bureau update (Officer Gaudet): On September 4, 2019, I made contact with the owner of LPS Enterprises, Tony Lisa. Lisa confirmed MA TRN: C98747 is registered to his company. Lisa stated he will look into who was using the trailer on 8/30/19 and will contact me with their information. I spoke with the operator of MV2, John Subilosky (S89884850). Subilosky stated he was operating a 2011 GMC Sierra (MA CON: T58930) with MA TRL:C98747 attached to it on the afternoon of the accident. Subilosky stated he was not aware he was involved in an accident and stated he would have stopped had he known. Subilosky stated there was a bobcat in the trailer at the time of the crash. Subilosky stated there is no damage to the trailer. Due to the size and weight of the trailer attached to MV2, it is reasonable to believe if MV2 made minor contact with MV1, the operator of MV2 would not have been aware. Subilosky has been added to this report as the operator of MV2.

On 9/08/19 Kevin Coston came into NPD Headquarters to report a MV/Ped with Injury that had occurred on 8/23/19 at about 06:15 in the parking lot of Whole Foods (916 Walnut Street). The RP stated that he had left work (Whole Foods) at the back receiving door and was walking to his car, he bent over to take his knee pads off, while he was doing this he was struck by a delivery van that backed into him. The driver took out his headphones and said sorry before driving away. RP stated that he told the driver that he had been hit but the driver just drove away. RP went to hospital (Faulkner) on his own for lower back pain and has not been to work since. RP was able to take picture of the side of the delivery van. The van belonged to Shortstop Distribution Inc (508-655-5948). RP left a message that morning. He was texted by Gedeon (786-352-6695) who stated there was a language barrier and he didn't understand what had happened. RP advised Gedeon to talk to his lawyer or the police. Exact location of accident unknown. Diagram is a estimate. Report printed out and placed in traffic box for follow up. Traffic Bureau Update (Officer Gaudet): On Monday, September 9, 2019 I attempted to make contact with the operator if MV1, Gedeon Duarte, with a negative result. A Hit and Run motor vehicle inquiry was mailed to Duarte. I spoke with the injured party, Kevin Coston, via telephone on September 9, 2019. Coston stated he is an employee at the Whole Foods located at 916 Walnut Street, Newton. Coston stated on August 23, 2019 a black van backed into him as he was kneeling on the ground taking off his knee pads in the loading area of the store. Coston stated he approached the driver who remained in the vehicle to speak with him after he was hit. Coston described the driver as a heavy set Spanish male. Coston stated the operator of the vehicle rolled down his window, took his headphones out of his ear, said sorry, and left the area. Coston stated he contacted the Newton Police to report the accident but was told he needed to come in person to file the report. Coston stated he was unable to come to the station to file the report until September 8, 2019. On Friday, September 13, 2019, I spoke with Gedeon Duarte at the Newton Police Department. Duarte stated he spoke Portuguese and did not speak English. With the assistance of David Sargent from the Language Line (1-800-225-5254) translating, I was to speak with Mr. Duarte in the Traffic Bureau. Duarte stated he was making a delivery at Whole Foods and was waiting for another truck to leave so he could leave the area. Duarte stated a male party (Coston) came up to his driver door and was yelling him. Duarte stated that due to language barrier he was unable to understand what the person was saying to him, but stated the male party was yelling, "Fuck you! Fuck you!" Duarte stated he did not know what was going on and stated he left the area to allow the male party to calm down. Duarte stated he returned a minute later the male party did not want to speak with him. Duarte stated he returned a second time later in the morning and Coston was not there. Duarte stated he received a call from his employer stating Kevin Coston had called for him and he was provided with Coston's phone number. Duarte stated he texted Coston to find out what happened that morning. Duarte showed me a text message exchange from August 23, 2019 at 9:03 A.M. between himself and a party listed as "Kevin Whole". In the text messages, Duarte stated he is the driver of the van and that his supervisor called him. The other party responded and stated "Talk to the police and my lawyer". Duarte stated that he did not understand what happened and that he returned to whole foods. Duarte ended the text exchange with "927535425 my number of policy..." Duarte stated he had no idea he made contact with Coston and stated at the time of the alleged accident he did not hear/see/feel anything otherwise he would have stopped. Duarte's information has been updated in this report to reflect him as the operator/vehicle involved. Duarte also submitted pictures of the rear of his vehicle, a 2008 black Ford E350 (MA: 8NY171), from the day of the accident. Photos were submitted to the IT Bureau. Officer Wade went to the Whole Foods located at 916 Walnut Street, Newton in an attempt to view surveillance footage of the accident. Officer Wade stated the surveillance camera did not capture the incident and the van not viewed at all.

On Tuesday, September 10, 2019 while assigned to Traffic unit N525, I responded to the area of Beacon Street and Roslyn Road, Newton for a report of a motor vehicle/bicycle crash. The weather at the time of the crash was overcast. The road surface was dry. Beacon Street and Roslyn Road are both public ways maintained by the City of Newton. I spoke with the bicyclist involved in the crash, Steven Greenzang (S96615185). Greenzang stated he was traveling Westbound on Beacon Street in the shoulder lane on his black Specialized bicycle. Greenzang had a yellow reflective vest on and was wearing a helmet at this time. Greenzang stated a vehicle in the travel lane to his left drove by him and hit the mirror located on his left handlebar. I observed damage to the mirror. Greenzang report no injuries. I spoke with the operator of MV1, Sarah Bonfanti (S66811372). Bonfanti stated she was parked in front of 1411 Beacon Street waiting to pick up a co-worker that resides at that address. Bonfanti stated a bicycle traveling on the sidewalk to her right crashed into the passenger side door mirror of her 2008 Toyota Scion (MA: 681YH1). Bonfanti stated the bicyclist was acting "crazy" during her interaction with him and he was trying to take her picture. Bonfanti stated she got back into her vehicle and moved up the roadway to wait for police to arrive on scene. I canvassed the area where the accident occurred with Sgt. Dempsey. There was glass from the passenger side mirror of MV1 and Greenzang's bicycle in the main travel lane of the roadway and the shoulder lane where Greenzang stated he was traveling. We also observed a large tree and a telephone pole on the sidewalk next to the roadway where the accident is alleged to have occurred. After speaking with all parties involved, observing the location of the glass in the roadway from the crash, and the location of the tree and telephone pole on the sidewalk where he accident occurred; Greenzang would not have been able to operate his bicycle on the sidewalk and be close enough to the roadway to make contact with MV1. Prior to clearing the accident, I asked Bonfanti if she had any questions and if she wanted to report any injuries. Bonfanti stated she was hit by the bicyclist during her exchange with him and began grabbing her left leg. I asked Bonfanti why she did not inform the officers on scene that she was assaulted. Bonfanti stated she said something to our Dispatch when she called. I asked Bonfanti if her leg was injured. Bonfanti stated I was putting words in her mouth and that she was just scratching her leg. Bonfanti then stated she was hit in the right shoulder. I asked Bonfanti if she would like an ambulance to respond and evaluate her. Bonfanti declined and stated she was more concerned about her mirror. I advised Bonfanti if she wanted to pursue charges against Greenzang she could do so in Newton District Court. I listened to the 9-1-1 recording Bonfanti made in regards to this incident. Bonfanti stated a pedestrian busted out her window. Bonfanti then stated to our dispatcher that the bicyclist lost control of his bicycle and smashed into her car. During her exchange with the our dispatcher, I did not hear Bonfanti say she was physically assaulted by Greenzang.

Operator of MV1 stated he was driving southbound on Lake Ave and looked down for a brief moment and did not see the bend in the road and struck the stone wall in front of 219 Lake Ave. The passenger of MV1 stated the operator was traveling southbound and did not see the turn and went up on the side walk and collided with the stone wall. A witness (Baptiste, Franck Jean) stated he witnessed the accident and believed the vehicle was travelling at approximately 40mph and lost control around the bend and struck the wall. David Donahue from Tody's Towing towed the vehicle. The homeowner at 219 Lake Ave was home and notified about the damage to their property. Six digital photos of the accident scene were taken and submitted to IT. Update 9/28/19: Upon further review and discussion with Sgt. Dempsey, Operator of MV1- Gallivan, Chase was issued citation T1445308- 89/4A- Marked Lane Violation and 90/8-JOL Passenger Restriction. Citation was issued in hand.

MV1 was parked in a legal spot facing southbound in the parking lot of 33 Austin St. MV2 was attempting to back up when he struck MV1. Operator 2 states he did not know he was so close to MV1 and backed up into him. No injuries. No tows needed. MV1 sustianed minor damage to the rear. MV2 sustained minor damage to the rear passenger side. 6 photos taken and placed into IT.

Katerina Iliades stated that on 10/23/2019 at 11:56 hours she was standing in front of the rear driver's side door of her vehicle MA reg 7BM656 2019 Land Rover while it was parked facing north in front of 316 Walnut St. Walnut St is a public way in the city of Newton. Iliades stated that she was placing her daughter Melina Bichelen in her car seat and when she observed MA reg 8EP212 a white Subaru Outback driving north bound on Walnut St near the marked crosswalk at 312 Walnut St. Iliades stated that she observed the Subaru was being operated by an elderly white male. Iliades stated that the she observed the Subaru's front drivers side strike several orange plastic three foot poles that are affixed to the roadway around the crosswalk. Iliades stated that after the Subaru struck the poles at the crosswalk it veered towards her. Iliades stated that she leaned back into the back seat of her vehicle and pulled the door towards herself as the Subaru's passenger side mirror made conact with her vehicle. Iliades stated that the Subaru continued north bound on Walnut St without stopping or slowing. Iliades complained of lower leg pain and refused medical aid or transport to a hospital at the crash scene. I observed no damage or markings on Iliades vehicle. MA reg 8EP212 is registered to Harilaos Papadogiannis ( DOB 01/02/1941 ) of 134 Apt 4 North St , Newton. I went to 134 North St and found MA reg 8EP212 a 2014 Subaru parked in the lot of 134 North St. I observed that the Subaru's passenger side mirror was lose from it's mounting and was turned down and was facing the wrong way. I further observed that the out side casing of the Subaru's mirror had scuff marks consistent with it forcefully making contact with an other object. I spoke with Papadogiannis who stated that he was driving his Subaru north bound on Walnut St at 11:56 hours on 10/23/2019. Papadogiannis stated that he could only recall striking the plastic poles near the crosswalk and nothing else regarding the crash. I as spoke with Papadogiannis he was confused and I had to repeat myself and my questions to him several times. I instructed Papadogiannis what steps he needs to take regarding filing out an operator crash report and contacting his insurance provider , but I believe that he did not fully comprehend what I was conveying to him. Based upon my observations at the crash seen and my conversations with Iliades and Papadogiannis , I believe that Papadogiannis was at fault for the crash. Based upon my observations of Papadogiannis I filed an Immediate Threat Request form and faxed it to the MA RMV Driver Control Unit. Photos of both vehicles were taken and downloaded by the NPD's IT Bureau.

On 11/12/2019 at 12:26 hours, I responded to the Star Market parking lot located at 2040 Commonwealth Avenue for a motor vehicle pedestrian accident. The parking lot is a public/thru way that motorists use to access the Star Market along with other businesses. The weather at the time was cloudy and rainy and the parking lot was saturated. Prior to my arrival, the pedestrian involved left the scene and did not want to speak to the officer nor wanted medical attention. Ofc. Claflin then cancelled Cataldo Ambulance. The pedestrian, Mary Alexander, left her information with a store employee prior to leaving. I spoke to the operator of MV # 1 (Joshua Randel MD Lic.# R534440603244) who stated that he was travelling through the third row of the Star Market parking lot at an appropriate speed. He was then attempting to make a left turn near the entrance/exit of the store. He says that there was a large white van parked in the end handicapped spot towards the left of him. He says the van was sticking out in the spot limiting his visibility. As he was half way through his left turn and coming around the corner, he was not able to see a pedestrian pushing a shopping carriage. He then struck the carriage that was being pushed by Mary Alexander. He further says that the cart jolted Ms. Alexander and she fell down to the ground. At the time, Alexander was walking through the lot near the store entrance heading back towards the parked cars. She was not in the crosswalk at the time which was approximately ten feet down from her location. After, I attempted to go to Ms. Alexander's residence to follow up. I spoke to her friend who was bringing up Alexander's groceries and she stated that she decided to go to Newton Wellesley Hospital for treatment. I then spoke to Ms. Alexander in the lobby of the hospital. She stated that she was exiting the store through the lot with her shopping carriage. She believes she was approximately eight feet away from the door when all of a sudden she only saw red coming at her. She believes MV # 1 was speeding at the time. I asked her if she was struck by the vehicle or was the shopping carriage only struck. She replied that she did not know and only remembers falling to the ground after. She sustained a large bruise to the inside palm area of her right hand and her left knee was swollen. Digital photos of the scene were taken and pictures of Ms. Alexander's injuries were taken at the hospital. As far as possible video surveillance, I was told that the Loss Prevention Department has to access the video. On 11/13/19 at approximately 14:20 hours, I went in to the Star Market and spoke to the manager, Nidal Rajeh, inquiring about possible video surveillance. He said there is coverage in the parking lot and he was able to show me the video. A copy of the video would have to be done by Loss Prevention which I requested. I observed at the 12:12pm mark in the video, MV # 1 was parked five spots up from the third row of the lot that he was exiting. I also observed a white transport van parked in the end handicapped spot mentioned by the operator but it was parked correctly and not limiting visibility. Next, MV # 1 backs out of the spot and drives forward going at a reasonable speed. The pedestrian (Alexander) has already exited the store with her carriage and is walking at an angle toward the same third row of parked cars. As the pedestrian is walking towards the direction of MV # 1, it appears that the pedestrian should be visible to him. MV # 1 then starts to take the left turn around the corner without slowing or stopping and strikes the carriage head on. Due to the impact, Alexander then loses grasp of the carriage and it goes past her twisting her body around. She then hits and rolls up on the hood of the vehicle on her left side. MV # 1 then suddenly stops and immediately gets out and provides assistance to the pedestrian. Due to the video surveillance, Mr. Randel will be issued/mailed MA Uniform Citation # T2012347 for Ch. 90/14/A Failure to slow for a pedestrian.

The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. The operator of MV1, Jessica Moore, stated she was backing out of a parking spot in the rear of the Angier Elementary School while operating a 2007 Chrysler Town and Country (MA:395KR9). Moore stated she did not see MV2 passing by as she was backing out and the rear driver side bumper area of her vehicle crashed into the rear driver side of MV2. Moore reported no injuries. I observed moderate damage to the rear driver side bumper area of MV1. The operator of MV2, Patrice Lys, stated he was traveling Westbound in the parking lot of the Angier Elementary school in a 2018 Ford EcoVan (MA MVN: M2546A). Lys stated a vehicle on his driver side backed out of a parking spot and the rear end of MV1 crashed into the rear driver side of MV2. Lys reported no injuries. I observed minor damage to the rear driver side area of MV2. The operator of MV1 was cited with Massachusetts Uniform Citation T2016428 for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care, Backing). Photos were submitted to the IT Bureau.

On 11/30/2019, while assigned to N494, I, Officer Conary, and other units, responded to Charlesbank Road at Nonantum Road for a single motor vehicle crash into a tree. Upon arrival, I observed a white mini van MA registration 5DV184 with heavy front end damage into a tree on Charlesbank Road, a public way in Newton. The operator of the motor vehicle, CORDERO, Ericka, was sitting in the back seat of the vehicle and was talking on the phone. CORDERO's primary language was Spanish. I asked CORDERO if she was okay and she said yes and I asked CORDERO if she was driving the car and she said yes. CORDERO also told me that she did not have a license. MA State Police were on scene and were able to asist with translating. Medics arrived and CORDERO was transported to Saint Elizabeth's Hospital. CORDERO's son and husband arrived on scene and I was able to confirm her information. Both stated that CORDERO did not have an active license. Newton Dispatch ran all information for CORDERO and confirmed she was unlicensed. CORDERO will be charged with MGL Ch.90/10 Unlicensed Operation of a Motor Vehicle and 89/4A Marked Lanes Violation, Uniform MA Citation T2079476. Due to CORDERO being transported to the hospital, the citation will be mailed to her active address. Pictures of the incident were taken and uploaded to IT accordingly. No further incident to report.

Reporting party parked her father's MV (MV1) infront of 4 Marlboro St (public way) facing northbound near the Tremont St intersection at approximately 1930 hours on Saturday 11/30/19. The weather at the time of the accident was light snow. The road surface was wet. Woodward Street is a public way maintained by the City of Newton. I spoke with the operator of MV1, Brad Butera (S94383997). Butera stated he was operating a City of Newton owned 2011 Volvo Front Loader (MA MVN: M88537) on Woodward Street while clearing the roadways during a declared snow emergency. Butera stated he was backing up Southbound across Woodward Street in the area of 364 Woodward Street after clearing some snow. Butera stated he looked and did not see any vehicles behind him. Butera stated he was backing the Front Loader up and the rear back of his vehicle crashed into the front hood area of MV2. I did not observe any obvious damage to the bucket area. Butera reported no injuries. Butera stated the operator of MV2 stated "My fault" to him during their exchange after the crash. Butera is an employee of the City of Newton DPW. I spoke with the operator of MV2, Marco Caceres (S67223904). Caceres stated he was operating his 2006 Mercedes E350 (MA: 26P270) Westbound on Woodward Street. Caceres stated he saw a Front Loader ahead of him clearing snow in the roadway. Caceres stated he stopped his vehicle in the roadway and then began backing into the driveway of his residence (Southbound) located at 364 Woodward Street. Caceres stated as he was backing into his driveway, MV1 began backing up towards him. Caceres stated he was honking his horn numerous time. Caceres stated the rear of MV1 crashed into the front hood area of his vehicle. I observed damage to the front driver side hood area. Caceres reported no injuries. I spoke with a witness to the crash, Emily Frederick. Frederick stated she was clearing snow off of her car in the driveway of 364 Woodward Street. Frederick stated she heard a car honking and looked up into the roadway. Frederick stated she observed MV1 crashed into the front end of MV2. Photos were taken of both vehicles and submitted to the IT Bureau.

Sgt. McNeil, N495, and I responded to 474 Wodward St. for an auto pedestrian accident. Upon arrival the victim, Elizabeth Cantwell was laying in the street and receiving treatment from NFD and Cataldo. The operator of Ma Reg. 192HV0 was Margaret Meehan. Meehan was visibly upset and was hearing impaired, as a result Meehan was unable to communicate what happened in the accident. At this time we were able to contact Meehans daughter, Bernadette Conley 781-248-6222. Cantwell was transported to Beth Israel Hospital. Traffic Bureau was notified. I went to the BI hospital to get a statement from Cantwell. Cantwell states that her husband parked their vehicle across the street from Starbucks. Cantwell states that she stood next to her car to cross the street. Vehicle 1 came to a stop and Cantwell believed that the vehicle was going to let her cross the street (not in crosswalk). Cantwell states that when she went to cross the street vehicle 1 hit her, causing her to land on the hood and then rolled off onto the street. At this time Cantwell has a dislocated hip and head injury. I took photos of the scene. Meehans vehicle was parked in a legal spot and I transported her to NHQ where she was picked up by her daughter Bernadette Conley. I filled and immediate threat with the RMV and a copy of the report was attached with the form.

Operator of vehicle one , Allan Wint stated on 01/27/2019 at 09:40 hours he was driving his employer's pick up truck south bound on Lexington St near number 227. Lexington St is a public way in the city of Newton. Wint is a driver for Auto Zone auto parts located at 140 Lexington St , Waltham, MA ( 781 894-0656 ) . Wint was driving a 2018 Chevrolet Colorado pick up tuck ( MA reg 8CA823 ) that Auto Zone leases from Lease Plan USA Ltd. Wint stated that he was headed to Webster St in Newton to pick up a delivery. Wint stated that as he approached the area of 277 Lexington St he believed that the traffic light that is at the marked crosswalk was green for south and north bound traffic. Wint stated that he did not see any pedestrians in the crosswalk, until he heard the sound of an object impacting into the front driver's side of his vehicle. Wint stated that he stopped his vehicle approximately ten yards past the crosswalk and got out. Wint stated it was then he realized that his vehicle had stuck a female and a dog. Wint stated that he was not operating any electronic devices at the time of the crash, and I observed none in his vehicle. Wint stated he was not injured in crash. Wint's vehicle had damage to the driver's side mirror and lower molding of the driver's side front door. Wint's vehicle did not require a tow. Witness Bessy Shanks stated that she was driving in her vehicle behind Wint's vehicle south bound on Lexington St. Shanks stated that Wint's vehicle was driving slow and swerving approximately two blocks prior to the crash. Shanks stated that she believed that Wint may of been distracted. Shanks stated that south and north bound traffic was stopped because that cross walk light was red and that a female and a dog were crossing from the east to west in the marked crosswalk in font 227 of Lexington St. Shanks stated that the front driver's side of Wint's vehicle srtuck the female and the dog as they were approximately halfway into the crosswalk. Shanks stated that the female and dog both fell to the ground after the crash. Witness Elizabeth Keegan stated that she was driving in her vehicle south bound on Lexington St two vehicle behind Wint's vehicle. Keegan stated that she was not sure what color the traffic light was at the time of the crash, but traffic was stopped both not and south bound traffic was stopped. Keegan also stated that Wint's vehicle " didn't even slow down ". Keegan stated that Wint's vehicle struck the female pedestrian and the dog as they were in the marked crosswalk in front of 277 Lexington St. Witness Henry Taylor stated that his vehicle was the first one stopped at the red crosswalk light at 227 Lexington St in the north bound lane. Taylor stated that north and south bound traffic was stopped and that Wint's vehicle did not stop and stuck a female pedestrian and a dog in the crosswalk. Witness Jacqueline Deos stated that her vehicle was stopped behind Taylor's north bound on Lexington St. Deos stated that north and south bound traffic was stopped for the red light . Deos stated that Wint's vehicle did not stop and that it struck a female pedestrian and a dog in the marked crosswalk. The pedestrian in the crosswalk was identified as Xiaoping Sun. Sun stated that she and her dog stopped on the south bound side of the marked crosswalk across from 227 Lexington St. Sun stated that she activated the crosswalk light and waited until the light was red for north and south bound traffic, and that the walk single was activated. Sun stated that she looked to her left and saw traffic north bound traffic was stopping. Sun stated that she turned to her right and entered the marked crosswalk. Sun stated that she and her dog had crossed over the north bound side of Lexington St and she then looked to her right and saw Wint's vehicle not stopping and " going very fast ". Sun stated that the front driver's side mirror of Wint's vehicle stuck her in the face . Sun stated that as she fell to the ground she was struck on the left leg by Wint's driver's side. Sun stated that she did not lose consciousness after she had fallen to the ground. Sun was transported via EMS to The Brigham and Women's Hospital for treatment for head and neck injuries and abrasions to her legs. Sun's dog died as a result of being struck by Wint's vehicle. Photo's of the crash site and of Wint's vehicle were taken and the disc was downloaded by the NPD's IT Bureau. Before leaving the crash site, I tested the crosswalk lights and they were in working order. Based upon my observations of the crash site, Wint's vehicle, and the statements made to me, I concluded that Wint was at fault for this crash. I issued Wint MA Citation T 2013958 and cited him for a violation of MGL 89/11 , failure to yield to a pedestrian in a crosswalk.

On 02/14/20 at approximately 14:45 I responded to a crash in the parking lot outside of #50 Winchester St- The Tile In Style store. Upon my arrival the Fire Department was already there and I was met by the owner of the building Bruno DiFazio. The vehicle that had struck the building had left the scene but he showed me security footage of the crash. The video showed a white S.U.V. (later determined to be Mass. Reg# 6GEL40 White 2017 Acura RDX owned and operated by Eleonora Revzin) pull slowly into a parking spot and then accelerate up and over the curb and into the building. The crash caused moderate damage to the brick facade and inside wall of the tile store. A short time later, Revzin drove back into the parking lot where I spoke with her. Revzin stated she didn't know why her car had accelerated into the building but she was having some personal issues and may have been distracted. She explained that she left the area because she was shaken up but then as she had time to clear her head, thought she should return to the store. There was minor damage to her vehicle and she stated she was not hurt and didn't want me to call the Medics for an evaluation. I issued Revzin Mass. Citation #T2013436 in hand for N.C.O. 19/75- Failure to use care in stopping and a verbal warning for leaving the scene. City of Newton Inspector Paul Nelson showed up and deemed the building to be structurally safe. N523 Officer Wilson arrived and took pictures of the scene.

On Tuesday, February 18th 2020 at 1908 hours, I responded to the area of 94 Vine St for a single motor vehicle accident. The vehicle (Mass Reg # 5FRT20, 2013 ELANTRA SEDAN COLOR BLUE) was being operated by William Lake. Lake was headed east on Vine St toward West Roxbury when he crashed into a City of Newton Tree. The road conditions were wet with heavy rain. Lake stated the wet conditions caused him to lose control of the vehicle. There was major front end damage to the vehicle and airbag deployment. Lake suffered a laceration to his left eye and complained of neck pain. Lake was transported to Newton Wellesley Hospital by the Medics. The vehicle was towed by Tody's. I took pictures of the accident and submitted them to the IT Bureau to be uploaded.

On Thursday 04/16/2020 at approximately 2306 hrs I was dispatched to a hit and run accident in the area of 320 Washington St. Upon arrival I observed debris all over the roadway including a City one way street sign, a City utility light pole along with a Metropolitan MWRA vent. The RP mentioned to dispatch he observed headlights driving away from the scene and believed it was a black vehicle once he exited his residence and observed the vehicle parts in the roadway.The vehicle later identified as a 2020 Toyota Rav 4 black in color bearing MA.REG (97E520) was not on scene of the accident. Shortly after I responded on scene the vehicle involved was located nearby by N-494 at the intersection of Washington St. at Adams St. The vehicle was registered to a Evgenii Shatokhin with heavy right front end damage along with the left front, and both rear tires deflated. Mr. Shatokhin stated he was the operator of the vehicle and I asked what happened back at 320 Washington St. He stated he was driving Westbound on Washington St. when he swerved to avoid something and hit the curb. Once he hit the curb, he stated he then hit the sign, light pole and MWRA vent. Due to Mr. Shatokhin not remaining on scene of the accident and leaving he is being charged with Ch. 90 Sec. 24 (Leaving the scene of an accident, property damage). Failed to remain on scene after accident occured involving property damage. He is also being charged with Ch. 89 Sec. 4A (Marked Lanes) for failure to drive entirely within. Citation #T2014768 was given in hand to Mr. Shatokhin. MV1 sustained heavy right front end damage. No injuries. Tody's responded on scene and towed MV1. A towed motor vehicle form was filled out and placed on file. Pictures were taken on scene of the property damage and placed into the IT Bureau's box.

On Wednesday, May 6, 2020 while assigned to Traffic unit N525, I responded to the area of Harvard Street and Washington Park, Newton for a report of a construction vehicle that crashed into a City of Newton fire hydrant. The weather at the time of the crash was clear and sunny. The road surface was dry. Harvard Street and Washington Park are both public ways maintained by the City of Newton. I spoke with the operator of the vehicle involved in the crash, Michael Clavette (S36731553). Clavette is employed S and P Construction and was operating a 1998 Mack 700 (MA CON: S71054) while working as a sub contractor for Feeney Brothers on Washington Park. Clavette stated he was attempting to back down Washington Park (W) from Harvard Street. Clavette stated he pulled his vehicle forward to reposition and the front bumper area made contact with a fire hydrant in front of 82 Harvard Street. I observed minor damage to the front bumper area of MV1. I observed minor damage to the fire hydrant. Photos were taken of the damage and submitted to the IT Bureau. The City of Newton Water Department was notified.

On 05/11/20 at 11:09 hours, I responded to the parking lot of All Star Liquors located at 1220 Chestnut Street for a two car accident involving a City of Newton vehicle. No injuries reported. On arrival, the vehicles had already parked back in to spots in the parking lot. I spoke to the operator of the city vehicle (MV# 1) identified as Brian Quiles who works for the City of Newton DPW Utilities Department. He was operating a 2019 Ford F550 dump truck color white bearing MA Official Tag# M3120A at the time. Mr. Quiles states he was operating eastbound attempting to exit the parking lot of 1220 Chestnut Street and turn left on to Oak Street. While attempting to exit the lot, he states that he stopped for a pedestrian crossing the street. At this point, he heard glass shatter but did not realize his truck was ever struck. He then got out and observed that MV# 2 backed in to the rear passenger side of his truck. There was no apparent visible damage to the city vehicle as a result of the accident. I then spoke to the operator of MV# 2 identified as John Deming who was operating a 2017 Honda CRV color green bearing MA reg.# RWCF30 at the time. He states he was parked in the last spot in the parking lot across from the All Star Liquors near the Oak Street entrance/exit. He then attempted to back up his CRV attempting to exit the lot. He says that he observed MV # 1 in the rear of him but thought he had enough room to back up around the truck. He says that he was using his back up camera and misjudged the distance and struck the passenger rear end area of MV# 1. As a result, his rear windshield shattered and he also sustained damage to his driver's side rear brake light cover area. Digital photos were taken of scene and submitted to the I.T. Bureau.

Operator of vehicle one, Waste Management employee Kevin Mellett, stated that on 06/25/2020 at 09:06 hours while driving south bound (MA Con reg V42947 a 2020 Mack Trash truck) his vehicle struck an unoccupied vehicle that was parked facing south in front of 21 Lewis St. Lewis St is a public way in the City of Newton. The parked vehicle was MA Con reg T37787 a 2014 Toyota pick up truck registered to AAA Sparkling Window Cleaning. Mellett stated that the mechanical arm that is used to pick up trash barrels was not in the fully closed position. Mellett stated that the his truck's mechanical arm was still partially extended and that the clamps on the arm were in the open position when he attempted to pass the parked vehicle. I told Mellett it was my understanding that the trash truck could not be put into drive as long as the arm was still extended. Mellett stated that the truck can be driven with the arm still partially extended. Vehicle one had no damage and vehicle two had damage to it's rear driver's side bumper and lower quarter panel. Neither vehicle required a tow, and I took photos of both vehicles. Based upon Mellett's statements I issued him MA citation T2015996 and cited him for a violation of City of Newton Ordinance 19/75, failure to use care while starting.

Operator of MV1 stated he was traveling Eastbound on California St. when MV2 crossed the double yellow lines and struck his vehicle. Operator of MV2 stated he was traveling Westbound on California St. approaching #150 when he attempted to slow down and stop, but stated his breaks didn't work and collided with MV1. Both MV3 and MV4 were parked facing Eastbound in front of 150 California St. at the time of the accident with no operators inside. MV1 collided with MV3 due to impact. MV2 collided with MV4 as well due to impact with MV1. with MV3 and MV4 due to impact. A scooter operator was also involved in this accident and stated he was traveling behind MV1 facing Eastbound and once MV1 was struck by MV2, he was forced into MV1's back left rear end. The scooter sustained minor front end damage. It should be noted MV2 struck MV1 at an angle where the vehicles came to a hault shown in the crash diagram provided in this report. An opposite direction sideswipe caused the vehicles to move in such direction. No injuries. MV1 sustained heavy drivers side damage. MV2 sustained heavy drivers side and front end damage. MV3 sustained minor left rear end damage. MV4 sustained minor left rear end damage. MV1 and MV2 were towed on scene by Tody's towing. A towed motor form was filled out for both vehicles. Operator of MV1 sent pictures along with a dash cam video of the accident to his insurance for review.

The truck's arm clamp was broken off in the crash. Owner of vehicle two Maura Brown stated her vehicle was parked unoccupied facing north in front of 19 Wood brine St prior to the crash. Neither vehicle was towed . Velasco stated that hen was not injured in the crash. I took photos of both vehicles and forward the disk to the NPD's IT Bureau for downloading. On 9/1/20 while working N493, I responded with N497 and the Fire Dept to 40 Crescent St for a report of a car into the house. On arrival I observed MA 6CM450 in the backyard of #40 unit C. It was stationary against the large AC Compressor in the rear of the unit, and appeared to had run over several large shrubs before coming to a stop. I spoke with vehicle owner and operator Marion Cohen 8/27/46. Cohen stated that as she was parking in the rear lot of the complex, she accidentally mistook the brake for the gas pedal. She in turn ended up going over the curb and through some bushes before hitting the compressor. There was a passenger in the vehicle identified as Milton Kornfeld 3/10/42. Both occupants were checked out by medics and declined transport. The airbags did not deploy. There does not appear to be any structural damage to anything other than the compressor. NFD checked the integrity of the house, and the compressor was unplugged. I took pictures of the scene and submitted the memory card to IT. Homeowner Xiaozhi Zhao and his family were present inside at the time of the collision and are unharmed. Tody's responded and removed the vehicle from the property. It sustained heavy front end damage and was towed to their lot via flatbed. A m/v inventory form was filled out. This report number has been provided to all parties for their respective insurance companies.

On 09/21/20 at 14:46 hours, I (Traffic Unit # 523) responded to the parking lot of 88 Chestnut Street for a motor vehicle accident involving a pedestrian. The weather at the time was 60 degrees ( localconditions.com) and the sky was clear and sunny. The road surface was dry. The parking lot is a thru way that the public use to enter/exit the lot or park there. On arrival at the same time with the Fire and Medics, I observed a female laying in the middle of the lot facing down on her back. A small red and gray CVS like type shopping carriage was knocked over and resting on it's side in front of the victim's head. The carriage was moved to the side in order to tend to the victim I also observed a black Honda Civic bearing MA reg.# 3741TW that was involved in the accident. The Honda was out of the parking spot resting approximately two feet away from the pedestrian's legs. I then spoke to the pedestrian identified as Darla Gipson in the rear of the ambulance. She sustained an abrasion to her front left shoulder and arm area which I observed. She also complained of left knee and elbow pain. She stated that she walking through the middle of the parking lot pushing the carriage by the door entrance in a westerly direction towards Chestnut Street. She states that she never saw a vehicle backing up and then she was suddenly struck and believes she was pushed underneath the vehicle. She was then transported to Newton Wellesley Hospital by Cataldo Ambulance. I then spoke to the operator of the Honda Civic identified as Linda Burke (MA S70320414). Ms. Burke was parked in the first spot to the right of the walkway that leads to the entrance/exit doorway. I asked her what occurred and she stated that she was backing her vehicle up to exit the parking lot when she looked behind her not seeing anything in sight. She then started backing up and struck the pedestrian without realizing it. She says the driver of a green cab and his female customer who were no longer on scene were yelling at her to stop her vehicle because the pedestrian was underneath it. She then stopped her vehicle. I observed minor scrapes to the entire rear bumper of the vehicle so it is unknown where it actually struck the pedestrian/carriage. I observed no obvious signs of damage to the carriage. It should be noted that Ms. Burke's Honda does not have a back up camera or back up sensors. There are no surveillance cameras to the apartment complex and no witnesses came forward for the accident. As a result of the investigation, Ms. Burke will be issued Uniform Citation # T1271069 for Newton City Ordinance 19-75 Failure To Use Care in Backing. Digital photos were taken by me of the scene.

On 10/22/20, while assigned to N499, I, Officer Conary, responded to 80 Redwood Road, for a report of a single vehicle accident into a tree. Upon arrival, I met with the Operator of MV's husband who was standing by at the car. Operator of MV1 walked home to contact her insurance company but she was on her way back to the accident. Operator of MV1 explained to me that she was traveling Northbound on Redwood Road to her house when she was adjusting the air conditioner. She stated she looked down twice to adjust it and then she hit the tree. Operator of MV1 declined medical attention. MV1 had heavy front end damage and was towed by Tody's. There was minor damage to the City of Newton tree. Pictures were taken and submitted to IT accordingly. City of Newton DPW was notified of the debris in the road and sidewalk.

On Tuesday, October 27th 2020 at 18:40 hours, Myself N-499 and Ofc Crowe N498 responded to 546 Parker St for a single vehicle accident into a city tree. Upon arrival, I identified Mass Reg # 3GDM90, 2015 LEXS RX350, Color Gray had crashed into a City Tree. The airbags had not been deployed. The operator was no longer in the vehicle upon my arrival. He was identified as registered owner Brian Mcneill. Mcneill admitted he had operated the vehicle. Mcneill stated he was driving North on Parker St toward RT 9 headed home to Wellesley. Mcneill stated he was on his phone texting and lost sight of the road. Mcneill's vehicle veered off the side of the road and side swiped a City of Newton Tree causing heavy right side damage and loss of the front right tire. Mcneill's wife came to the scene to pick him up and drive him home. The set of golf clubs in the trunk of the vehicle was documented in the MV Inventory Crash Form. Mcneill suffered no injuries and signed a patient refusal with the medics. Tody's towed the vehicle. Mcneill was given MA Uniform Citation # T2013042 for the amount of $ 210 for M.G.L Ch 89/S4A Marked Lanes Violation and M.G.L Ch. 90/S13B Texting While Driving Violation. Pictures were taken of the accident and submitted to Newton IT Bureau.

On 11/11/2020 I, Officer Mildner, was dispatched to 48 Windermere Rd. for a report of Gas Lamp being struck by a single MV. Operator of MV1 stated that he was traveling Northbound on Windermere Rd when he took a left and pulled into the driveway of 47 Windermere Rd. in an attempt to back out and turn around. Operator of MV1 stated that as he was backing out Eastbound, he hit a pole containing a gas lamp at the top, right in front of 48 Windermere Rd. Witness 1 stated he saw Operator of MV1 pull into the driveway of 47 Windermere and back out directly into a pole containing a gas lamp at the top. There were no injuries. No damage to MV1. There was minor damage to the light pole and gas lamp. Fire was successful in turning off the gas. Pictures were taken and submitted to the IT Bureau.

Operator 1 stated that he was parked in a designated parking space in the lot of 259 Centre street when he viewed mv#2 backing up and getting close to mv#1. Operator #1 stated that he beeped his horn to alert mv#2. Mv#2 then struck the front passenger side of mv#1. On Wednesday, December 02, 2020, while assigned to Traffic unit N525, I was traveling Westbound through the rear parking lot of 1321 Washington Street at approximately 0905 hours. As I was traveling through the lot, I observed Parking Control unit 1308 backing (S) out of a parking spot ahead of me to my right. Parking Control unit 1308 is a City of Newton owned vehicle (MA MVN: MP1308). I stopped my vehicle in the roadway to allow 1308 to continue backing out of their parking spot. As 1308 continued to backing, the rear driver side bumper area of the vehicle crashed into the front passenger side bumper/fender area of Parking Control unit 1309 (MA MVN: MP5E). Parking Control unit 1309 was parked (W) in the roadway unoccupied. I observed no damage to either vehicle. The operator of Parking Control unit 1308 exited her vehicle and was identified as Parking Control Officer Angela Foley. PCO Foley stated she was backing her vehicle out of its parking spot and did not see 1309 parked behind her. PCO Foley stated the vehicle was in her blind spot. PCO Foley was asked if she was injured as a result of the crash. PCO Foley stated she was not. Photos were taken of both vehicles to document the crash and submitted to the IT Bureau.

On Friday, December 4, 2020 while assigned to Traffic unit N525, I responded to the area of 34 Etsy Farm Road, Newton for a report of a crash involving a City of Newton contracted Waste Management truck. Etsy Farm Road is a public way maintained by the City of Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. I met with the operator of MV1, Cameron Jacques (S82958796). Jacques stated he was operating a 2014 Mack 600 (MA CON: R46465) southbound on Etsy Farm Road. Jacques stated there were two vehicles parked on opposite sides of the road as he was attempting to continue straight past both vehicles. The rear passenger side tire area of his vehicle made contact with the rear bumper area of the vehicle to his left (MV2). MV2 is a 2010 Lexus RX450 that was parked unoccupied facing northbound in the southbound lane across from 34 Etsy Farm Road. The rear bumper of MV2 was damaged and removed from the vehicle as a result of the crash. The registered owner of MV2, Pine Cone Construction, was advised. Jacques and his passenger Jose Lux de Leon reported no injuries. Photos were taken of the damage to both vehicles and submitted to the IT Bureau.

Cragmore Road is a public way maintained by the City of Newton. The weather at the time of the crash was cloudy. The road surface was wet from melting snow in the area. The operator of MV1, Jon Papia, stated he was operating City of Newton Fire Engine 7 (MA MVN: MF470 ) Southbound on Cragmore Road. As MV1 travelled past 8 Cragmore Road, it's passenger side made contact with the front driver side area of MV2 (MA: 1JTC19). The Honda Accord was parked and unoccupied at the time of the crash. The registered owner was made aware of the crash and advised. I observed no damage to the passenger side of MV1. I observed damage to the front driver side of MV2. Photos were taken and submitted to the IT Bureau. No injuries were reported by any of the involved parties.

At approximately 1655HRs on Monday, January 25, 2021, I was dispatched to the 15 Comm Ave. for a report of an MVA, in which the offending vehicle left the scene. Upon arrival, I spoke with the owner of MV1 (NH Pass: 2873301), who stated that his coworker ( Witness 1) observed MV2 hit his vehicle and leave. To this end, he showed me the resulting damage to vehicle's rear bumper, as well as a photograph Witness 1 took of MV2. Boston Police located MV2 at it's listed garaged address. Upon speaking with the owner/operator of MV2 (MA Pass: 17MA21), he stated that he was in the area, but denied hitting MV1. I informed him of the witness statements and asked him about the damage to his vehicle, but he again stated that he did not believe he was involved, yet could not account for the damage. Upon speaking with Witness 1, he stated he was walking Westbound on Comm Ave. at approximately 1650HRs, when he observed M2 attempt to exit a parking spot in front of 15 Comm Ave. (MV2 was parked behind MV1). MV2 then scrapped MV1 while attempting to do so, causing MV1 to rock forward. Witness 1 then took a picture of MV2's rear license plate. I observed scratches to the rear driver's side bumper area of MV1 and scratches to the front passenger's side bumper area of MV2, along with a slight separation of the bumper cover of MV2 and it's mounting points on the same side. Both vehicle's damage was consistent with Witness 1's account of a vehicle scraping a parked vehicle, as it attempted to exit a parking spot from the rear. It should be noted that the damage to both vehicles appeared minimal and it is plausible that the operator of MV2 may not have known about the collision. As a result, the operator of MV2 will not be charged with a hit and run accident at this time. I took pictures of both vehicle's damage and they, along with Witness 1's picture will be submitted to IT to be attached to this report.

On 2/12/2021, at 2315 hrs, Newton Police were dispatched to the area of Watertown St. and Brookside Ave. for a single motor vehicle collision. Upon arrival, MV#1 (Red, Jeep Grand Cherokee) was observed parked, on the side of the road facing eastbound in front of 687 Watertown St. MV#1 was observed to have front end damage and two flat tires (driver side). The operator, who was standing outside the vehicle, stated he was traveling eastbound on Watertown St. when he became distracted, lost control of the vehicle and collided into the snow bank.The driver did not report any injuries. The vehicle was towed from the scene by Perfecton Towing of Waltham, Ma. Damage to MV#1 does not appear consistent with the operators recollection of events, but a check of the immediate area by Newton Officers did not yield any damaged property. Damage to MV#1 was photographed with the (N492) camera and will be attached to this report.

The operator of MV#1 stated she was in the parking lot of 330 Homer St (Newton Free Library) attempting to park into a parking spot when her vehicle all of the sudden accelerated, went over the burm striking MV#2 (Parked and unoccupied), striking MV#3 (Parked unoccupied) and pushing MV#3 into MV#4 (Parked and occupied with 4 people). The operator of MV#1 stated she was not distracted and her vehicle just accelerated forward by itself. The front passenger of MV#1 stated they were attempting to park into a parking spot when the vehicle accelerated on its own. MV#1 sustained moderate damages to its front end. There were no reported injuries to the operator of MV#1 or its passenger. MV#1 was deemed mechanically sound and left the scene after being provided with the necessary information pretraining to this accident. MV#2 was parked next to MV#3 and was unoccupied when it was struck by MV#1. MV#2 sustained minor damages to its front passenger's side bumper. The owner of this vehicle was provided with the necessary information pertaining to this accident. MV#3 was parked in between MV#2 and MV#4 and was unoccupied. MV#3 was struck by MV#1 which pushed MV#3 into MV#4. MV#3 sustained heavy front end damages (Broken driver's side axle). The operator of MV#4 was inside his vehicle along with 3 other passengers when they were struck by MV#3. MV#4 sustained heavy damages to its driver's side rear door. There were no reported injuries to the operator of MV#4 or its 3 passengers. The witness stated he was inside his vehicle a few parking spots over from MV#4 and observed MV#1 drive over the burm striking MV#2 then striking MV#3 and pushing MV#3 into MV#4. MV#3 sustained the heaviest damage and was inoperable. MV#3 was towed by AAA Towing Services. I took digital photographs of the damages sustained to the city's burm.

On 03/30/21 at 09:58 hours, Ofc. Marini (N537) and I (Ofc. Wilson (N523) responded to the parking lot of Tedeschi's located at 612 Washington Street for a two car motor vehicle accident involving a city motor vehicle which belonged to the Newton D.P.W.. At the time of the accident, weather conditions were clear and sunny with dry road conditions. The operator of M/V # 1, Danielle Finklea MA D/L S09835151, states she was eastbound on Washington Street in her 18' Mitsubishi Outlander (MA reg. # 9TM745) and turned right in to the parking lot of Tedeschi's. While pulling in, Finklea misjudged the distance that M/V # 2 was parked in a spot to the left of her. She then struck the right side of M/V # 2's rear bumper which sustained scrape marks to it. The Outlander sustained scrapes to the driver's side front quarter panel area and a broken headlight lens cover to that same side. The other vehicle (M/V # 2) was a 2014 Ford F350 pickup bearing MA Official Reg.# M87258 which is owned by the City of Newton Department of Public Works. At the time, the F350 was parked in a parking spot in front of the store unoccupied. The operator, Robert Mazzarella, stated he saw the F350 shake as it had been struck so he walked over to the other operator involved. No injuries reported. Ofc. Marini took digital photos of the scene and submitted them to the I.T. Bureau.

On Friday, April 2, 2021, I spoke with Mr. Michael Maguire from the City of Newton Buildings Department in the Traffic Bureau. Mr. Maguire wanted to report a crash the he was involved in this morning with his City of Newton owned vehicle. Mr. Maguire reported the crash occurred at 612 Washington Street, Newton. Mr. Maguire stated he was traveling (E) through the 7-11 convince store parking lot at approximately 0755 hours this morning. Mr. Maguire stated he was operating a City of Newton owned 2010 Ford Ecovan (MA MVN: M99243) at the time of the crash. Mr. Maguire stated at this time a vehicle to his right backed out of it's parking spot and it's rear driver side bumper area crashed into the middle passenger side of his vehicle. I observed minor damage to the middle passenger side door area of MV1. Mr. Maguire reported no injuries. Mr. Maguire reported MV2 was a 2015 Honda Civic (MA: 588LG8) and the vehicle was being operated by Mr. Donald Sampson (S29010431). Mr. Maguire stated Mr. Sampson exchange information with him and he observed damage to the rear driver side bumper area of MV2. Mr. Maguire provided me with a picture of the damage to MV2. I have submitted it to the IT Bureau to be added to this report. I took picture of the damage to MV1 and submitted them to the IT Bureau.

On Tuesday, April 20, 2021 while assigned to Traffic unit N525, I responded to the area of 1188 Centre Street for a report of a crash involving a Toyota Prius and City of Newton owned vehicle.. Centre Street is a public way maintained by the City of Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. The operator of MV1, Herbert Kepnes (S88523672), stated he was operating his 2008 Toyota Prius (MA: 77VF98) at the time of the crash. Kepnes stated he was attempting to exit the metered parking spot he was park in directly in front of the Brookline Bank entrance on Centre Street. As Kepnes attempted to merge into traffic, his vehicle's front passenger side crashed into the driver side fender/door area of MV2. Kepnes and his passenger, Sally Kepnes, reported no injuries. I observed moderate damage to the front passenger side of MV1. The operator of MV2, Stephen Dempsey, was operating a City of Newton owned Ford Expedition (MA MVN: 505B) South on Centre Street near the Brookline Bank entrance in his official capacity as a Newton Police Officer. Sgt. Dempsey stated has he was traveling on Centre Street, a vehicle to his left exited a metered parking spot and attempted to merge into traffic in front of him. Sgt. Dempsey was unable to avoid making contact with MV1. MV1 crashed into the front driver side fender/door area of MV2. I observed damage to the front drive side door/fender area. Sgt. Dempsey reported no injuries. Photos were taken of the vehicles and submitted to the IT Bureau.

Motor vehicle 1 (MV1) was parked facing northbound in the parking lot of 98 Winchester Street ( Tedeschis), when an unknown motor vehicle 2 (MV2) crashed into the rear passengers side of MV1, and then left the area. It is unknown if MV2 backed into MV1 or hit MV1 head on. As a result of the crash, MV 1 sustained moderate passengers side rear end damage. An unknown witness told the owner of MV1 that the vehicle involved was a dark colored VW sedan with a MA registration of 826PHL, however, this registration number does not come back to a VW. The Tedeschis does have cameras facing the parking lot, however, the employee working at the time of the crash did not have access to them and advised that the manager would be able access the footage tomorrow. On Monday, May 3, 2021, I investigated a Hit and Run crash that occurred at 98 Winchester Street, Newton on May 2, 2021 (Crash#2100000303). I responded to Tedeschi's located at 98 Winchester Street, Newton in an attempt to recover surveillance footage of the crash. I spoke with the owner who allowed me access to review footage from the date and time of the crash. At approximately 1223 hours a black/dark blue Volkswagon sedan is observed entering the Tedeschi's parking lot from Dedham Street (N). The vehicle is observed taking a wide left turn around another vehicle that enters the parking lot from the Winchester Street entrance (E). The front passenger side bumper/fender area of the Volkswagon crashes into the rear passengers side of MV1 (NY: HXZ4415). The Volkswagon is then observed leaving the parking lot (W) out of the Winchester Street side of the lot. The Volkswagon then leaves view of the camera. The Volkswagon's back rear window has white lettering on the bottom. I searched CopLink for registration matches from the Newton area that contain characteristic from the reported registration provided by a witness. MA: 826PH4 comes back to a blue Volkswagon Passat registered to Ms. Joanne Medoff (S95046822) with an address out of 115 Upland Avenue, Newton. This address is 0.1 miles away from where the crash took place. I arrived at 115 Upland Avenue and observed a dark colored Volkswagon Passat (MA: 826PH4) parked in the driveway. This vehicle had moderate recent damage to it's front passenger side bumper/fender area that would be consistent with the crash from Tedeschi's. This vehicle also had the same white lettering present on the bottom of the rear window as the vehicle in the surveillance footage. I rang the doorbell to this address and spoke with Mr. Colby Medoff (S13831145). Mr. Medoff stated the vehicle in the driveway is his but it is registered to his mother. Ms. Medoff came to the door and I explained the circumstances for me speaking with them. After speaking to Mr. Medoff, he admitted to being the operator of the Volkswagon when it crashed into the rear passenger side of MV1. I asked Mr. Medoff why he did not stop and provide his information to the owner of the other vehicle. Mr. Medoff stated he was scared. Mr. Medoff was provided with Massachusetts Uniform Citation 990226AA in hand for Chapter 90, Section 24 (Leaving the Scene of Property Damage). Mr. Medoff was advised. Photos of the Volkswagon and media from the Tedeschi's surveillance footage were submitted to the IT Bureau. Mr. Medoff's information was added to this report to reflect him being the operator of MV2. I contacted the party in possession of MV1, Mr. Jason Medley, and advised him. The parking lot of Tedeschi's is private property but the public does have a right of access to the property.

On Friday, May 7, 2021, while assigned to Traffic unit N525, I responded to the area of Colby Road for a report of a crash involving a City of Newton owned vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with Mr. Michael Sanford from the City of Newton Water Department. Mr. Sanford stated his City of Newton owned vehicle (2013 Ford F-250 (MA MVN: M88493) was parked unoccupied (S) in front of 497 Centre Street, Newton. Mr. Sanford stated his emergency lights were on while he was marking out a service. Mr. Sanford stated he then observed a white 2005 Ford Ecowagon (MA CON: L61558) travel past his vehicle South on Centre Street and make contact with his driver side door mirror. Mr. Sanford stated the vehicle continued down Centre Street and he followed it to Rochester Road where is came to a stop. I observed damage to the driver door mirror. I spoke with the operator of MV2, Ms. Jerrie Spencer (S12241050). Ms. Spencer stated she knew she made contact with MV1's driver door mirror, but was trying to make it to her appointment at the Faulkner Hospital and that is why she did not immediately pull over. I observed damage to the front passenger side door mirror. Ms. Spencer was provided with Massachusetts Uniform Citation 994181AA for Chapter 89, Section 4A (Marked Lanes Violation). Centre Street is a public way maintained by the City of Newton. Photos were taken of both vehicles and submitted to the IT Bureau.

On Monday, May 17, 2021 I responded to the parking lot entrance of the Avalon at Chestnut Hill (160 Boylston Street, Newton) for a report of a motor vehicle/pedestrian crash. The weather at the time of the crash was clear and sunny. The road surface was dry. Boylston Street is a public way in the City of Newton. I spoke with the operator of MV1, Mr. Joshua Champion (SA1310472). Mr. Champion stated he was operating his 2013 Lincoln MKS (MA: VT41980) while attempting to exit the Avalon parking lot at the Boylston Street (E) off ramp. Mr. Champion stated he was monitoring the traffic to merge and as he started to make his right turn his vehicle crashed into the left side of a pedestrian in the roadway. Mr. Champion stated he did not see the pedestrian as he was making his turn. I observed no damage to the area where the pedestrian made contact with the car. Photos were taken and submitted to the IT Bureau. The injured pedestrian, Mrs. Joan Contompasis, stated she was walking on Boylston Street (W) with her husband. Mrs. Contompasis stated she entered the roadway from off the sidewalk to cross the Avalon parking lot exit and continue on Boylston Street. Mrs. Contompasis stated she believed MV1 had stopped to allow her and her husband to cross. Mrs. Contompasis stated the vehicle then accelerated and it's front passenger side bumper area crashed into her left side. Mrs. Contompasis fell to the roadway. Mrs. Contompasis was transported to Newton Wellesley Hospital for injuries to her right foot and knee. I spoke with a witness to the crash, Mr. Michl Contompasis. Mr.Contompasis stated he was walking with his wife Joan on Boylston Street (W). Mr. Contompasis stated he was walking just ahead of his wife as they crossed the Avalon at Chestnut Hill exit at Rt. 9. Mr. Contompasis stated he observed a black sedan to his left as he crossed. Mr. Contompasis stated he believed the operator was not paying attention as he exited the parking lot and the vehicle accelerated into his wife's left side.

On 06/14/21 at approximately 12:00 hours, Sgt. Wade received an email from Nina Wang (D.P.W. Transportation Traffic Superintendent) that a city utility trailer with a signboard attached to it was struck by another motor vehicle at 275 Brookline Street sometime over the weekend between 06/11/21 and 06/14/21. A photo was sent over in the email of the utility trailer and signboard knocked over on it's side over the sidewalk. The utility trailer is 2020 Wanco color orange bearing MA Official registration # M9994A. The trailer was parked in the bicycle lane of the northbound travel lane next to 275 Brookline Street. The signboard was there in an attempt to get motorists to drive the speed limit due to the Memorial Spalding School located at 250 Brookline Street. On 06/14/21 at approximately 12:00 hours, I responded to Brookline Street to photograph the trailer and possibly process the scene. The trailer could not be located. I learned later that the trailer had already been towed by Tody's towing and brought to the City D.P.W. yard located at 110 Crafts Street. On 06/15/21 at approximately 09:00 hours, I went over to 110 Crafts Street and was told that the trailer/signboard was in the garage. The trailer was already being repaired to be placed back in to service. It had sustained damage to the two rear brackets where the jacks are located and the license plate bracket and registration plate were bent. The signboard message frame board is no longer aligned properly as a result of the accident but reportedly still functions. After, I then responded back to 275 Brookline Street and took digital photos of the scene. I observed in the bike lane at the accident scene two small pieces of lens covers to a vehicle with no identifying marks. I also did a canvass for possible surveillance cameras in the area. There was no response at number 269 and 275 Brookline Street but I left a note for them to call me. 283 Brookline Street has no surveillance cameras. Memorial Spalding School is across the street from the accident and I observed a camera on the side of the building which faces the staff parking lot on to Brookline Street. I spoke to a staff member Jesse Brackett at the school. He called David Stickney (Superintendent of Schools) and notified him of the accident. Mr. Stickney was provided with my email and will send over any possible footage if he has it. At approximately 12:45 hours, I spoke to Mike Sylvester who resides at 275 Brookline Street. He stated that he had surveillance footage and emailed it over to me. I viewed the footage which is a camera that views his approx. 75 foot driveway leading out to Brookline Street. There is a fence and bushes behind where the signboard is located which limits it's view. The video shows the accident took place on 06/11/21 at 18:48 hours. At this time, you can see the trailer is struck and slowly falls over on it's side. Immediately after a white colored four door sedan continues to slowly drive past the scene northbound on Brookline Street in the bicycle lane without stopping. Two other vehicles behind the white sedan drive around to the left of the white vehicle at this point. In the footage, you can not view the registration number, operator details, or the type of sedan from the footage. On 06/16/21 at 09:40 hours, I spoke to David Stickney who called the Traffic Bureau. He stated that he had footage of the accident but it did not reveal any other details besides the same white sedan striking the trailer.

On Wednesday, June 16, 2021 while assigned to Traffic unit N525, I responded to the front entrance of the Brown Middle School located at 125 Meadowbrook Road in Newton for a single vehicle crash involving a City of Newton contracted school bus. It was also reported by Newton Dispatch that one of the occupants on the bus had suffered an arm injury as a result of the crash. Meadowbrook Road is a public way maintained by the City of Newton. The road surface at the time of the crash was dry. The weather at the time of the crash was clear and sunny. When I arrived, I was updated by Officer Iarossi that the crash involved a City of Newton contracted school bus transporting students from the Williams School (141 Grove Street, Newton). The students were participating in "Step Up" day and were leaving Brown to go back to Williams. Officer Iarossi stated a student on the bus was injured as a result of the crash. Tegan Quirk was transported by Fallon Medics to Newton Wellesley Hospital with a right arm injury. The remaining students that were on the bus reported no injuries to staff at the time and were moved to another bus to be transported back to the Williams School. I asked the Brown staff that was assembled in front of the school to contact the Williams School and ask them to hold the children involved in the crash at the school once they arrived. I made this request so we could make sure there weren't any new injuries reported; and I wanted to inquire if any of the children witnessed the incident. The staff stated they would contact Williams and relay the request. I spoke with the operator of the school bus (MA BUN: BU44536), Mr. John Aubert (S48613114). Mr. Aubert stated he was operating bus #361 while transporting students from Brown Middle School to the Williams School. Mr. Aubert stated as he was preparing to leave Brown, he made an announcement to students on the bus to sit down. Mr. Aubert stated he got the okay to leave from one of the staff members on the bus. Mr. Aubert stated there was another bus parked in front of him prior to leaving. Mr. Aubert stated he began to reverse the bus to create distance from the bus in front of him, and then operated the bus forward to exit from his position on the right side of the road and merge into the travel lane to his left around the bus parked in front of him. Mr. Aubert stated as he merged onto the roadway, he heard screaming from the students on the bus and the rear passenger side of the bus made contact with a brick support column in front of the school positioned along the sidewalks edge next to the roadway. Mr. Aubert stated he exited the bus and observed a student's arm pinned on the rear passenger side of the bus between the side of the bus and the brick column. Mr. Aubert stated he got back in the bus and reversed it until the students arm was free. Mr. Aubert stated at this time, staff helped the injured student. The staff there helped facilitate transportation for the remaining students back to the Williams School. Mr. Aubert reported no injuries, but was concerned for the welfare of the student. After speaking with Mr. Aubert, I observed damage to the school bus and the brick support column as a result of the crash. I observed fresh damage (scrapes) to the rear passenger side of the school bus between the last passenger side window and the back of the bus. I also observed fresh damage to the brick support column. The column is a support to an overhead covering that runs from the sidewalk to the front entrance of the school. There was fresh paint transfer from the bus and a crack in the column. Pictures were taken of the damage and submitted to the IT Bureau. The travel lane where the crash took place is a one way access road that runs from Meadowbrook Road to the front of the school, and exits onto a parking lot between the Brown Middle School and the Oak Hill Middle School. I responded to the Williams School to follow up with staff there regarding this crash. All of the students that were involved in the crash had been released prior to my arrival. I spoke with the Principal of the Williams School, Ms. Ayesha Farag, Ms. Michal Shapiro (Social Worker), and Ms. Katie Costello (School Nurse) about the students on the bus. They stated no injuries were reported to them, and they notified the parents of the involved parties about the crash. They also provided me with contact information for all of the students on the bus at the time of the crash. I spoke with the three teachers that were on the bus at the time of the crash, Ms. Kimberly Andersen, Ms. Tess McLean, and Ms. Katherine O'Leary. All three teachers stated prior to the bus leaving Brown Middle School they accounted for all of the students and were ready to leave to travel back to the Williams School. One of the teachers recalled hearing the bus driver make an announcement to the bus that they were about to leave, but could not recall the exact wording of the announcement. The teachers stated as the bus began to move, they heard screaming from the rear of the bus, looked back, and saw a student's arm stuck outside the window. After realizing the student was injured, they assisted with helping to free the student from his pinned position. They helped the injured student until EMS arrived on scene. They assisted the remaining students by directing them away from the crash and onto another bus for transportation back to Williams. The three teachers reported no injuries as a result of the crash. I responded to the Newton Wellesley Hospital Pediatric Emergency Room to check on the status of the injured student, Tegan Quirk. I spoke with Tegan and his mother, Kathryn Quirk in his room. While attempting to talk to Tegan and his mother, Tegan became very upset and did not want to talk about the specifics of the crash. Out of courtesy for Tegan, I exited the room and spoke with his mother outside the room. I explained to Ms. Quirk the events of the crash, and how her son's arm became injured. I asked Ms. Quirk if she could assist me with faciliating a conversation with Tegan about the events of the crash. I explained to her I understood how traumatic the events of the crash were for her son, but as part of my investigation I would need to find out what led to Tegan's arm being outside the bus window at the time of the crash. While speaking to his mother, Tegan explained that he was waving to a friend with his arm out of the bus window when the bus started to move. Tegan stated his arm hit the column, and became stuck between the bus and the column. Tegan stated he was screaming for the bus to move and that it took forever for the bus to move so he could free his arm. Tegan stated the bus driver gave no warning to the students that the bus was going to start moving. The staff at Newton Wellesley Hospital stated Tegan has a right arm injury. The severity of the injury is unknown at this time due to further examination that needs to take place. I advised Ms. Quirk about the conversations I had with the other parties involved. I spoke with Mr. David Stickney (Head of Facilities) from the Newton Public Schools to try and obtain footage of the crash. Mr. Stickney stated unfortunately there is not footage of the crash from the Brown Middle School or Oak Hill Middle School surveillance cameras. I advised Ms. Regina Moody (Newton Public Schools Transportation Director) about the crash. Ms. Moody was able to show me footage of the incident from the camera inside the bus. In the footage, the bus driver can be heard announcing to the students prior to the bus moving, "Guys, guys, get off the seat and sit correctly". One of the teachers can be seen doing a count of the students. The teacher is heard saying to the student, "Everyone needs to be in a seat and sitting at 3 and 2 and 1 and thank you." Student are observed still standing up. The teacher is then heard saying, "sit down sit down." After the teacher passes the students in the back of the bus, she says, "We are good." to the front of the bus. Students are then observed standing after she passes. As the bus begins to back up, students can be observed standing in the rear of the bus and not sitting down on their seats. As the bus moves forward, a student (Tegan Quirk) can be seen standing on his seat and extending his right arm out of the last window on the passenger side of the bus. Screaming can then be heard as his arm makes contact with a support column on the passenger side of the bus. The bus is stopped, the driver exits the bus to asses what happend, gets back on the bus, and reverses the bus a short distance. The injured students right arm is released and he brings his arm back into the bus through the window. The amount of time between the students arm being pinned against the column to when it is released is approximately 40 seconds. After speaking with all parties involved and reviewing the footage from inside the bus, it appears that Bus #361 was pulling out of it's parked position in front of the Brown Middle School to enter the roadway and return students to the Williams School. A student that was sitting in the last row on the passenger side of the bus extended his right arm out of a window next to his seat to say goodbye to a friend. While his right arm was extended out of the rear passenger side window of the bus , the bus collided with a brick support column affixed to the sidewalk in front of the school. When the bus made contact with the column, the students right arm made contact with a brick support column and became pinned between the column and the passegner side of the bus. After the students arm was freed from it's pinned position, the student was able to bring his arm back into the school bus and was transported to Newton Wellesley Hospital for a right arm injury.

On 06/21/2021 at approx. 0545 hrs I was at the Yard at 52 Elliot Street speaking with other officers. I was parked between cruisers N500 and N498. A large bee flew into the window of N507 and was in the area of my face. I was unable to open the cruiser door so I attempted to pull the cruiser forward and did not realize the wheels of the cruiser were angled and I hit the back passenger side corner of N498 causing damage to the light and the rear quarter. The damage to N507 was the passenger side front and rear door. Photos were taken and submitted to the IT department. No injuries were sustained. Both cruisers were able to be driven.

Opr of MV1 reported a possible hit and run. I spoke to the Opr of MV1 he stated he was attempting to park in a parking spot in front of Los Amigos on Walnut Street. At this time, he was angled in to park and observed the vehicle (MV2) behind him attempt to pass him. While passing, MV2 made contact with MV1. MV1 suffered minor damage and was not towed. Opr of MV1 thought they were stopping, but the did not and took off towards Austin Street. A witness, Xavier Vacquez observed the accident and followed MV2. Xavier took a photo of the license plate and sent it to the Opr of MV1. Through an inquiry, I made contact with the Opr of MV2. Opr of MV2 stated she was traveling on Walnut Street behind MV1. She observed MV1 park and then pull out a little and was not sure what MV1 was doing. At this time, MV2 passed MV1 and drove to the parking lot. Opr of MV2 told me she thought she was struck in the parking lot and did not realize she made contact with MV1 back on Walnut Street. I explained there would be an accident report and advised her of the process. After an investigation it was determined it was not a hit and run. MV2 suffered minor damage and was not towed. There were no injuries reported.

Mr. Drukman stated the crash occurred on Monday, August, 2, 2021, at the Jewish Community Center located at 333 Winchester Street, Newton sometime around 1600 hours. Mr. Drukman stated his vehicle was parked unoccupied in the parking lot and was struck by another vehicle. Mr. Drukman stated his 2017 Subaru Forester (MA: WA1/UIY) had damage to the driver side rear fender. Mr. Drukman stated a passersby forwarded pictures of the other vehicle involved in the crash. The other vehicle is a 2016 BMW 5 series (MA: 1691DR) registered to Mr. Richard Parker (S75189008). Mr. Drukman forwarded me the pictures he received and there is damage to the front passenger side bumper/fender area of this vehicle. These photos were submitted to the IT Bureau to be added to this report. I spoke with Mr. Parker via telephone. Mr. Parker stated he was operating his vehicle in the parking lot of the JCC at the time and date of the crash. Mr. Parker acknowledged his vehicle made contact with another vehicle while he was attempting to park. Mr Parker stated after making contact with the other vehicle, he exited that parking spot because it was too tight and parked in another parking spot. Mr. Parker stated there is no new damage to his vehicle and the damage I described in the pictures Mr. Drukman forwarded to me was old. I advised Mr. Parker that I was citing him for leaving the scene of property damage. Mr. Parker admitted to crashing into MV1 as he was attempting to park in the JCC parking lot. Mr. Parker knowingly made contact with another vehicle and made no effort to exchange information with the owner of the vehicle he crashed into. The pictures submitted by Mr. Drukman of his vehicle show obvious fresh damage as a result of a crash. After completing my investigation, Mr. Parker will be mailed Massachusetts Uniform Citation 089044AB for Chapter 90, Section 24 (Leaving the Scene of Property Damage). The public does have a right of access to the parking lot for the Jewish Community Center located at 333 Winchester Street, Newton.

On Wednesday, September 1, 2021, while assigned to Traffic unit N525, I responded to the Newton Centre Municipal Parking Lot (797 Beacon Street, Newton) for a report of a motor vehicle crash involving a pedestrian with a potential head injury. The weather at the time of the crash was cloudy and overcast. The road surface at the time of the crash was dry. The Newton Centre Municipal Parking Lot is open for public travel and use and is maintained by the City of Newton I spoke with the operator of the 2016 Audi 32 (MA: 4PL345) involved in the crash, Mr. Peter Schiller (S26414610). Mr. Schiller stated he entered the Newton Centre lot from Langley Road and made a right turn to enter the back row of parking stalls. Mr. Schiller stated he then took a left turn into the parking area to look for a spot. Mr. Schiller stated he did not see the pedestrian in the roadway in time and his vehicle's front end crashed into the pedestrian. Mr. Schiller estimated the crash happened right near the first car parked in the row closest to where he entered. I showed Mr. Schiller the believed area of impact was actually 3 car stalls in, and the crash occurred in the middle of the travel way. I asked Mr. Schiller if he has any medical conditions or takes any medication that would effect his ability to operator a motor vehicle. Mr. Schiller stated he does not. Mr. Schiller reported no injuries as a result of the crash. I observed very minor damage to the front hood/bumper/fender area of his vehicle from where the crash took place. I did observe soil covering the hood area. The injured pedestrian was carrying a plant with soil in the pot. I asked Mr. Schiller if he could recall where he made contact with the pedestrian or how the pedestrian ended up in front of his vehicle. Mr. Schiller stated he thought the injured pedestrian was running towards his car when the made his turn. I spoke with a witness that remained on scene, Mr. Nicholas Ryland. Mr. Ryland stated he was walking in the Newton Centre lot while pushing his daughter in her stroller. Mr. Ryland stated he observed MV1 enter the lot from Langley Road and took the right turn into the lot at a high rate of speed. Mr. Ryland stated he stopped with the stroller next to the first vehicle parked in the roadway where the last parking stalls begin. The MV1's driver side operated past Mr. Ryland's left side, took a left turn, and then Mr. Ryland stated he heard the crash. The crash took place in the middle of the roadway in between the parking stalls to the East and West of MV1. I responded to Brigham and Women's Hospital in Boston to follow up with the pedestrian injured in the crash, Ms. Sophia Alexandridies (S49399467). Ms. Alexandridies was transported to the emergency department by Fallon Medics with multiple injuries including a head injury. Due to Ms. Alexandridies's injuries, I was unable to obtain a statement from her about the events of the crash. I spoke with Ms. Alexandridies's daughter, Christina, in the emergency department. Christina stated Ms. Alexandridies suffered a head injury, along with possible broken ribs and an injury to her left shoulder. Most of the injuries reported are on Ms. Alexandridies's left side, the presumed area of impact with the vehicle. Due to the circumstances of this crash, and Mr. Schiller being involved in another surchargeable crash on April 28, 2021, a Request for Immediate Threat License Suspension was submitted to the Registry of Motor Vehicles at DCUImmediateThreat@massmail.state.ma.us. Mr. Schiller is also being cited with Massachusetts Uniform Citation 119853AB for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). On September 2, 2021, I contacted Mr. Schiller to provide him with additional information about the crash. Mr. Schiller stated he did not realize Ms. Alexandridies was injuried in the crash. Mr. Schiller also stated he has no recollection of being involved in a crash in April. Mr. Schiller was advised to contact his insurance company to inquire about his last crash. Photos were taken of the area and MV1 and submitted to the IT Bureau. I canvassed the area for survellance footage of the crash with a negative result.

I spoke to the homeowner of 15 Walsingham St identified as Joonho Choi who stated to me the following: On or about 8:48 AM this morning a Jeep Grand Cherokee was turned into his driveway in order to turn around on Walsingham St and in the process, the MV struck his stone wall causing moderate damage. The driver of the MV identified himself to the homeowner and they exchanged paperwork. I advised Choi I would document the crash for him. There is unknown damage to the MV due to the fact that the driver had already left prior to my arrival and no phone number was provided. Choi has the accident recorded on his surveillance cameras and was advised to keep all pictures and videos for insurance purposes. The driver of the MV is identified as Bruce Roberts (4/2/55).

On Tuesday, September 14, 2021, while assigned to Traffic unit N525, I responded to 82 Lincoln Street, Newton for a report of a motor vehicle crash involving a City of Newton owned vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. Lincoln Street is a public way maintained by the City of Newton. The operator of MV1, Maureen Doucette, stated she was operating a City of Newton owned 2015 Ford Focus (MA MVN: MP1308) on Lincoln Street (E). Ms. Doucette was operating this vehicle in her official capacity of a Newton Parking Control Officer. Ms. Doucette stated as she was traveling past 82 Lincoln Street, MV2 attempted to exit their parking spot on the right side of the road and merge into MV1's travel lane. MV2's front driver side bumper/fender area crashed into the middle passenger side of MV2. The operator of MV1 reported no injuries. The operator of MV2, Nancy Greenwald, stated her 2012 Honda Accord (MA: 2EGT90) was parked in a parking spot in front of 82 Lincoln Street. Ms. Greenwald stated while operating her vehicle she attempted to merge onto Lincoln Street (E) from her parking spot. Ms. Greenwald stated as she made her left turn into traffic, the front driver side of her vehicle crashed into the passenger side of MV2 as it passed by. Ms. Greenwald reported no injuries. On Thursday, September 23, 2021, while assigned to Traffic unit N525, I responded to the area of 358 Beacon Street, Newton for a report of a motor vehicle/bicycle crash with injury. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of the vehicle involved, Mr. Thomas Mogan (S29907615). Mr. Mogan stated he was operating his 2015 Jeep Wrangler (MA: 6EBH20) on Beacon Street (W) towards his residence of 358 Beacon Street. Mr. Mogan stated he observed a break in the traffic traveling on Beacon Street (E) towards Hammond Street in front of his driveway. Mr. Mogan took a left turn from the Westbound lane of Beacon Street to enter his driveway. While making his turn, his vehicle crashed into a bicyclist traveling on Beacon Street (E) in the marked bicycle lane in front of 358 Beacon Street. Mr. Mogan stated he did not see the bicyclist until the front passenger side fender/door area of his vehicle made contact with her. I observed minor damage to this area on his vehicle. Mr. Mogan reported no injuries. The injured bicyclist was identified as Ms. Louisa Palmer (S67185637). Ms. Palmer was treated on scene by Fallon Medics for injuries to her left wrist and ankle and transported to Brigham and Women's Hospital for further evaluation. Ms. Palmer was wearing a helmet at the time of the crash and I observed her blue Bianchi Pista bicycle resting on the sidewalk in front of 358 Beacon Street. The bicycle had minor damage in to it's front wheel area and fresh scrapes near the handlebars. I transported the bicycle to Ms. Palmer's residence. I responded to Brigham and Women's Hospital in Boston and spoke with Ms. Palmer in the Emergency Department. Ms. Palmer stated she was riding her bicycle on Beacon Street (E) towards Boston College. Ms. Palmer stated she was riding in the marked bicycle lane with the motor vehicle traffic to her left. Ms. Palmer stated she observed a break in the traffic on Beacon Street ahead of her, and then a Jeep entered her travel lane after it took a left turn from the Westbound travel lane on Beacon Street right in front of her. Ms. Palmer stated her left side crashed into the front passenger side wheel/hood area. Ms. Palmer stated she then fell to the roadway. Ms. Palmer stated she has injuries to her left wrist/arm area, her left ankle, and her hips. I spoke with a witness to the crash, Ms. Kelli McCusker. Ms. McCusker stated she was traveling on Beacon Street (E) towards Boston College at the time of the crash. Ms. McCusker stated there was heavy traffic in the area as she observed a bicyclist travel by the passenger side of her car. Ms. McCusker stated there was a break in the traffic ahead of her in the Eastbound lane and she observed a Jeep in the Westbound lane of Beacon Street make a left turn in front of the car in front of her. Ms. McCusker stated the Jeep and the bicyclist crashed into each other at this time. After speaking with all parties involved, Mr. Mogan did not yield to the right of way while Ms. Palmer was traveling on Beacon Street (E) on her bicycle. Mr. Mogan will be issued Massachusetts Uniform Citation for Chapter 90, Section 14B (Fail To Yield To Bicyclist on Turn). Photos were taken of the scene and submitted to the IT Bureau.

On 10/01/2021, while assigned to N494, I, Officer Conary, responded to Centre Street and Hyde Ave for a report of a single vehicle accident. Upon arrival, I met with operator of MV1, who was outside of the vehicle. There was significant damage to the left front tire and front end. Operator of MV1 explained to me that she was traveling Northbound on Centre Street and next thing she remembered was being pulled over on the side of the road and not being able to drive the vehicle. Operator of MV1 stated she was not traveling fast because there was another vehicle in front of her. At this time, medics were called on scene for an evaluation. I was able to determine that MV1 was traveling Northbound on Centre Street, veered off to the right, struck the curb, which caused the tire to pop, and then the front end grazed the utility pole. Tody's towed the vehicle. Operator of MV1 was transported to the hospital. There was minor damage to the Verizon Utility Pole. No structural damage. Verizon was notified. The city was notified to clean debris in the road. Pictures of the utility pole were sent to IT to be uploaded accoridingly.

On 10/19/21 at 08:30 hours, Sgt. Wade notified me that he received an email regarding a motor vehicle accident involving a pedestrian/student that occurred on 10/15/21 between the hours of 08:20am and 08:35am. The crash was never reported by the operator of the vehicle or the student/pedestrian. Approximately two hours later, the student went to the nurse's office seeking assistance for neck pain which is how the school became aware. This crash occurred in the marked crosswalk on Brandeis Road in front of Newton South High School which is located at 139 Brandeis Road. The weather at the time was 63 degrees with mostly cloudy and sunny conditions (wunderground.com). Brandeis Road is a two way roadway divided by an unprotected median that runs east and westbound. In front of the school there is a marked crosswalk in the roadway. There is also a one way driveway near the main entrance of the school with parking stalls and parents can drop their children off there on the right side of the driveway. The driveway then leads back out to Brandeis Road. Right after you exit the lot there is the above mentioned crosswalk to the left of the driveway which is very visible with nothing obstructing your view. On arrival, I spoke to the Dean of the school named Charlie Myette. Mr. Myette brought me to his office and started looking at the school's video surveillance system. He also requested that the student/pedestrian involved to report to his office. The student was identified as Tsukasa Iinishi. I asked Mr. Iinishi what had occurred and he stated that he was walking to school on the morning of 10/15/21. He says that he was wearing a pink hoody sweatshirt with gray shorts. He was walking through the woods which is across the street from the school which exit out to Brandeis Road. He says that he waited at the crosswalk and let two vehicles go westbound by him. He then thought it was clear to proceed and started walking. At the tail end of the crosswalk he was struck by a red colored Toyota Prius which exited left from the driveway and was on the left side of the roadway. He was struck on the left side of his body and he says that he used his left hand to brace himself against the hood and that's when the Prius stopped. He says that the operator of the Prius asked him if he was okay and he replied that he was. He then kept walking toward the school. The operator then continued on it's way without providing any information or notifying us. He described the operator as an Asian female with black hair approximately 40 to 50 yrs in age. During this time, Mr. Myette was able to find surveillance video of the crash. There are two camera angles. One shows the driveway along with the walkway of the school building. The other one shows the same but it also overlooks out in to Brandeis Road. The view of the road is partially blocked from the parked cars in the lot. The video shows a red colored Toyota Prius pull in to the driveway of the school. An Asian female student wearing a white long sleeve shirt with green lettering and dark colored jeans exits the passenger front of the Prius and eventually enters the school. The Prius then pulls out and goes toward the left side of the driveway around other vehicles waiting to exit. At 08:27 hours, the Prius pulls out angling towards the left side of the driveway going around other vehicles waiting to exit. The Prius then exits the lot on the left side (wrong side) of the roadway and makes contact with the pedestrian. In the video you cannot see the front hood area of the car, but you see Iinishi's torso bend towards the vehicle on impact. It shows the Prius stopped there for approximately ten seconds and then it drives away. In the video you are not able to see the registration plate number. Mr. Myette is going to send the photo of the student dropped off in the Prius to his staff to have her identified so I can follow up with the operator. On 10/21/21, I received an email from Mr. Myette stating that some of his staff has not come forward on the identification of the student. He stated that he would now send a message out to a larger group of staff to see if he gets a response. He also attempted to enhance the registration plate using the surveillance software but the plate is still too blurry to make out. On 10/25/21 at approximately 08:40 hours, I went over to Newton South High School to see if I could locate the red Toyota Prius involved in the crash. As I was driving on Olde Field Road approaching Brandeis Road near the school, I observed a red Prius approximately four cars ahead of me in heavy school traffic. Eventually, I observed the Prius pull in to the drop off driveway of the school and drop an Asian female student off. I identified her as the same student that got out of Prius prior to the past accident on 10/15/21. The operator was an Asian female which appeared to be in her late forties. After, the Prius exits the lot on to Brandeis Road and I flagged her down. I had her pull in to the parking lot across the street from the school. I asked her if she was involved in a accident with a student on the morning of 10/15/21 to which she replied that she was. She was identified as Xiaojin Liu through her MA Driver's License # S49033927. She was operating a 2010 Toyota Prius bearing MA reg.# 8JY630. I asked her what occurred and she stated that she was exiting the driveway of the school to turn left on to Brandeis Road. She says that she has a habit to think that no one is ever walking through the crosswalk and thought it was clear to proceed. She turned and then saw the student crossing and attempted to stop but was not able to in time. She then bumped the student with the front of her vehicle. She says that she stopped and asked the student if he was ok several times and he stated that he was. The student then walked away and she says that she did not know what else to do. I advised her that she should of gave her information to the student, notify the school, or call us to which she understood. I issued MA Uniform Citation # 172129AB to Ms. Liu for Ch. 90/24/A Leaving the scene of personal injury. After, I went inside the school and notified Mr. Myette of my findings. I asked him for the surveillance footage as I previously did in person and through email. He stated that he would put the video on a zip drive and notify me.

On 10/21/21 at 15:24 hours, I (Traffic Unit 523) responded to 1479 Washington Street for a motor vehicle accident involving a bicyclist. The weather at the time was seventy three degrees with mostly cloudy skies and some sun(weather.com). The road conditions were dry and Washington Street is a public way maintained by the City of Newton. Washington Street is a two way divided roadway with an unprotected median that runs in a easterly and westerly direction. At this location, there is a marked crosswalk with a walk signal along with traffic signals above in both directions. On arrival, the scene was cleared up and both parties were waiting in the parking lot of the Burke & Blackington Funeral Home at 1479 Washington Street. I spoke to the bicyclist involved who was identified as Rajmattie Thakoordeen. I asked her what occurred and she stated that she was attempting to cross Washington Street in a southerly direction on her electric bicycle. She says that she pushed for the crosswalk signal to activate and once it did she began riding her bicycle when she was suddenly struck by a silver Saab 93. She made it out in to the crosswalk approximately four feet when she was struck by the Saab which was travelling westbound in the right lane of Washington Street. She says that she was struck on the left side of her leg area. She complained of her left leg being sore near her knee area but signed a patient refusal with Fallon Ambulance declining transport. At the time, she was riding a Rad Power Bike Roadrunner 1 Plus that is silver and black in color. It is a Class 2 electric bicycle (750W) that can go up to 20 m.p.h. The only damage it sustained was scrapes to the outside left pedal. Ms. Thakoordeen was wearing black/gray/white pants, yellow reflective jacket, and a black bicycle helmet at the time. Next, I spoke to the operator of the Saab 93 bearing RI registration # FN193. He was identified as Nevan Hanumara with Rhode Island Driver's License # 9960501. He stated that he was travelling westbound on Washington Street in the right travel lane when he came around the curve and noticed the red light signal above in the last second. He says by the time he reacted and attempted to stop he went through the red light and made contact with the bicyclist in the crosswalk. He reported no injuries and no damage to his vehicle. Due to his statement and inattention to the traffic signal, I issued him MA Uniform Citation # 168985AB for Chapter 89/9 Fail to Stop/Yield for the red traffic signal. Digital photos were taken of the scene.

On 11/03/21 at 08:25 hours, N491 and I (Traffic Unit # 523) responded to 612 Washington Street for a two car motor vehicle accident involving a city motor vehicle. The weather at the time was clear and sunny and the road conditions were dry. Washington Street is a public way maintained and owned by the City of Newton. On arrival, I spoke to the operator identified as Brian Napolitano (MA D/L #S73156918) and his co-worker Kevin Brooks who was a passenger in the vehicle. Both parties work for the City of Newton Department of Public Works. Mr. Napolitano was operating a 2009 Ford F350 bearing MA MVN registration# M78129 at the time. He stated that he was attempting to back out of the driveway of 612 Washington Street. Once it was clear to proceed he backed out on to Washington Street and that is when a Subaru Impreza made contact with his driver's side front quarter panel area. He says the Subaru was making a left turn from the left westbound lane of Washington Street in to the same driveway he was backing out of at the time. There was no visible damage to the vehicle. I then spoke to the operator of the Subaru (MA reg.# 3GYT41) identified as Jennifer Shippie (MA D/L# S23299202). Ms. Shippie states she was in the left westbound lane of Washington Street and was turning left in to the driveway of 612 Washington Street. She further stated that a vehicle in the eastbound lane stopped for her and waved her on to proceed. She then made the turn and never saw the Ford F350 backing out and made contact with it on her passenger front bumper area. The entire vehicle had damage to the front bumper consisting of scratches and dents but she stated none of it was new from this accident. No injuries reported and digital photos taken of the vehicles by me.

The operator was identified as Robert Lawton (MA D/L # S92175566). Mr. Lawton works for Commonwealth Construction and Utilities which is owned by the Distefano Corporation in Watertown, MA. They are contracted with the City of Newton to remove debris such as concrete from the city yard. At the time, Mr. Lawton was standing on a ladder attached to the trailer overseeing it being loaded up by a city backhoe. The truck was parked and facing in a westerly direction. During the loading process, the backhoe was pulling forward towards the truck in a westerly direction. The front left side wheel of the backhoe struck the truck's driver's side rear mud flap area denting it. The mud flap is made of metal and covers the four rear tires from front to back. Both Mr. Lawton or the operator of the backhoe never realized the truck had been struck until another worker from Commonwealth Construction noticed it. The backhoe is a yellow 2014 JCB 3CX14 with MA Apportioned Reg. # M88532. The operator of the backhoe Michael Daniele (MA D/L # S43438211) works for the City of Newton Dept. of Public Works out of 74 Elliot Street. There was no damage to the backhoe. Digital photos taken.

On November 30, 2021, N524(Ofc. Iarossi) was parked unoccupied at 292 langley Rd(north bound). Motor vehicle 2 Ma Reg 60760 was driving north on Langley Rd. MV2 was traveling/passing too close to N524 and the side view mirrors hit. Ofc. Iarossi was able to stop the MV at 210 Langley Rd and identify the operator. Lawrence Seder stated that he heard something but did not know what had happened. Ofc. Iarossi took pictures and submitted them to IT.

On Friday, January 7, 2022, while assigned to Traffic unit N525, I responded to the area of Commonwealth Avenue and Auburn Street, Newton for a report of a City of Newton plow truck that knocked over a traffic signal and left the area. The weather at the time of the crash was overcast and snow. The road surface was wet. Commonwealth Avenue and Auburn Street are both public ways maintained by the City of Newton. On arrival to this area, I observed a traffic signal resting on the sidewalk in front of 1897 Commonwealth Avenue. The traffic signal was cracked at it's base, and still functioning while resting on the sidewalk. The traffic signal when operation faces traffic traveling Westbound on the Commonwealth Avenue carriage road at Auburn Street. An anonymous resident of the area approached me and showed me a picture on their phone of the truck that left the area after crashing into the traffic signal. The photo was of a large City of Newton orange dump truck with a plow affixed to the front of it. I could not make out the registration of the vehicle, but I could see the number 48 on it's side to identify it. I requested Newton Dispatch contact the DPW to identify who the operator of the vehicle was. A message was left for Mr. Perry Cacciola (Newton DPW) for information about the crash. On Monday, January 10, 2021, I spoke with Mr. Cacciola. Mr. Cacciola stated Donald Leblanc (S00405419) was the operator of the plow responsible for the damage to the traffic signal. Mr. Cacciola stated he could not provide any information about the vehicle used in the crash, except that it was number 48. I then spoke with Mr. Leblanc. Mr. Leblanc stated he was operating the City of Newton plow involved in the crash. Mr. Leblanc stated the snow at the time of the crash was packed down and the plow made contact with the traffic signal while he was clearing the snow in front of 1897 Commonwealth Avenue. Mr. Leblanc stated he contacted his supervisor, Perry Cacciola, sometime after the the incident to reprot the damage. Mr. Leblanc reported no injuries. Photos were taken of the damaged traffic signal and submitted to the IT Bureau.

On Thursday, January 20, 2022, while assigned to Traffic unit N525, I responded to the area of 1446 Beacon Street, Newton for a report of an MVA involving a City of Newton contracted school bus. The weather at the time of the crash was light rain. The road surface was wet. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1, Mr. Ralph Ghera (S43446773). Mr. Ghera stated he is a driver for JSC Transportation (contracted by the City of Newton) and was picking up a student to transport them to Newton South High School. Mr. Ghera stated his vehicle, a 2019 Ford Transit Van (MA SPN: 109403 ), was parked in front of Beacon Street (E) with it's red flashing lights activated while Ms. Ella Glennon was entering the vehicle for transport. At this time, MV2 (2020 Lincoln Aviator (MA: 3AB494) travelled past MV1 Eastbound on Beacon Street and disregarded the flashing lights that were activated on the van. MV2's front passenger side door mirror made contact with MV1's front driver side door mirror. I observed very minor damage to the mirrors on both vehicles. No injuries were reported. I spoke with Ella's father, Ian Glennon, on scene and made him aware of his daughters involvement in the crash. Mr. Glennon stated his daughter was not injured. Ella was then transported to Newton South by Mr. Ghera. The operator of MV2, Mr. Yipei Zhang (S13848781), was issued Massachusetts Uniform Citation 265793AB in hand for Chapter 90, Section 14 (Fail to stop for a school bus). Photos were taken and submitted to the IT Bureau.
