Narrative:
On 04/08/2019, at 1620 hrs, I responded to the area of 2040 Commonwealth Ave for a three car MVA with injuries. Upon arrival, I observed all three vehicles facing westbound on Commonwealth Ave. MV1 had heavy front end damage and airbag deployment. MV2 also had heavy front end damage and airbag deployment. MV3 had minor rear end damage. The operator of MV1 was brought to Newton Wellesley Hospital by Cataldo medics. The operator of MV2 stated she was on Pamela Ct attempting to take a left onto Commonwealth Ave. There was heavy traffic headed westbound and a vehicle stopped to let MV2 go. As MV2 was taking a left onto Commonwealth Ave, MV1 was traveling westbound in the eastbound lane around traffic and struck MV2. MV2 then in turn struck MV3s rear end. The operator of MV3 stated that he wasn't sure what occurred because the accident was behind him. The operator of MV1 stated she was traveling westbound on Commonwealth Ave and was planning on taking a left into the Star Market parking lot which was approximately 250 feet down the street. She stated she thought there were two lanes in the westbound direction at that point in the roadway. MV2 then pulled out in front of her and they collided. Two witnesses who were traveling westbound in traffic a few vehicles behind MV3 both stated that MV1 drove erratically into the oncoming lane and around traffic heading westbound. One witness stated "the car was almost completely in the opposite lane" and the other witness agreed. A moment later they stated they heard the crash but did not see it. They also stated that she almost crashed into a car turning eastbound out of the Star Market exit prior to the accident. MV1 and MV2 were towed from the scene by Tody's. The operator of MV2 was brought to Newton Wellesley Hospital by her mother. The operator of MV1 was cited for C90 S23 operating after revoked license, C90 S23 operating after revoked registration, C90 S24 operating to endanger, and C89 S4a marked lanes violation. All aforementioned streets are public ways maintained by the City of Newton.

Operator of Motor Vehicle #1 stated that he was traveling Eastbound on Jackson Road and preceded straight through the green light at the intersection of Jackson Road and Pearl Street and was struck by motor vehicle #2 that ran the red light. Operator of Motor Vehicle #2 stated that he was traveling Northbound on Pearl Street and the glare of the sun was in is eyes and did not see the red light until it was to late and struck motor vehicle #1 which caused it to roll over. Operator of motor vehicle #2 stated that he helped to operator of motor vehicle #1 get out of the car and stated that he had a few scratches on his knee. Motor vehicle #1 sustained major damage throughout the entire vehicle. Operator of Motor vehicle #1 sustained minor injuries and was transported to Newton Wellesley Hospital. Motor vehicle #2 sustained moderate front end damage. Tody's Towing towed motor vehicle #1. A citation was issued to the operator of motor vehicle #2 for failure to stop at red light..

DRIVER STATES SHE WAS TRAVELING E-BOUND ON MILL ST. WHEN SHE WAS HIT IN THE RIGHT SIDE OF HER VEHICLE B VEHICLE #2. DRIVER STATES THE HIT SPUN HER COMPLETELY AROUND. VEHICLE #2 SPED AWAY FROM THE SCENE N-BOUND ON UPLAND RD. OPERATOR OF VEHICLE #1 REPORTS NO INJURIES. WAS ADVISED TO CONTACT HER INSURANCE COMPANY. AT THE ACCIDENT SCENE WAS THE LICENSE PLATE ( MA REG#, 1MB926 ). IT HAD COME OFF VEHICLE #2 DURING THE CRASH. I HAD DISPATCH RUN THE PLATE AND IT CAME BACK TO ( MARTIN EPSTEIN, DOB: 7-31-42, WATERTOWN ,MA. ). PRIOR TO CALLING THE OWNER OF THE PLATE I WAS NOTIFIED BY DISPATCH THAT THE PARTIES INVOLVED IN THE ACCIDENT WERE AT THE STATION AND WOULD LIKE TO SPEAK TO ME. UPON ARRIVAL AT THE NEWTON POLICE STATION FRONT DESK I MET ( MARTIN EPSTEIN JR). HE STATED THAT HIS MOTHER ( MARSHA EPSTEIN) ALSO PRESENT WAS OPERATING THE VEHICLE THIS AFTERNOON WHEN THE ACCIDENT OCCURRED. HE STATED SHE WAS ON HER WAY BACK ROM BETH ISRAEL HOSPITAL ONCOLOGY WHERE HER HUSBAND ( MARTIN EPSTEIN ) WAS DIAGNOSED WITH A GLIOBLASTOMA. SHE WAS INFORMED THAT IT WAS FATAL AND THEY WOULD BE PLACING HIM IN HOSPICE CARE SOON. EPSTEIN JUNIOR STATED HIS MOTHER CALLED HIM BELIEVING SHE WAS IN AN ACCIDENT BUT DOES NOT REMEMBER THE DETAILS. I INFORMED THE PARTIES ABOUT WHAT HAD OCCURRED FROM THE INFORMATION I HAD RECEIVED FROM THE OPERATOR OF VEHICLE #1. I INFORMED BOTH PARTIES THAT SINCE THEY CAME IN I WOULD NOT BE SEEKING COMPLAINTS FOR LEAVING THE SCENE OF PROPERTY DAMAGE. THEY WERE INFORMED THAT THEY WILL BE RECEIVING A CITATION FOR ( 89/9, FAILURE TO STOP FOR STOP SIGN ). I ESCORTED MARTIN EPSTEIN JUNIOR TO THE REAR OF THE STATION WHERE I RETURNED THE FRONT PLATE THAT WAS FOUND AT THE SCENE. HE INFORMED ME THAT SINCE HIS MOTHER DOES NOT RECALL THE ACCIDENT HE HAS TAKEN HER KEYS AWAY AND REVOKED HER DRIVING PRIVILEGES FOR THE SAFETY OF ALL PARTIES INVOLVED. HE STATES HE HAS SIBLINGS AS WELL AS FRIENDS OF THE FAMILY WHO LIVE IN THE AREA AND STATED THEY WOULD DRIVE HER WHENEVER NECESSARY. I INFORMED MS EPSTEIN THAT I WOULD MAIL HER A CITATION ( T1445179, 89/9, FAILURE TO STOP AT STOP SIGN ). IN CONSIDERATION OF ALL FACTS INVOLVED AN IMMEDIATE THREAT WAS FILED AND SENT TO THE REGISTRY OF MOTOR VEHICLES ON 9-29-19.

On Wednesday 12/4/2019 at approx 2110hrs, while assigned to N-494, I responded with the medics to 22 Park St Apt #5 in Newton for a past MVA pedestrian. There I was met by Annie Peng who states that at approx 730pm this evening she was struck by a vehicle(mass reg 6258) at the intersection of Washington St and Park St. She states that when she was entering the crosswalk after crossing over the mass pike bridge, she was struck from the right passenger side of MV1. Peng states she fell a few feet from the car injuring her right knee and right arm. She said that the operator stopped and got out to assist and left his contact info. Peng signed a patient refusal form and pictures of her injuries were taken and submitted to IT. It should be noted that Peng believes her cell phone was broken after hitting the ground. I later spoke with Dr. Ross, operator of MV1, who stated that he had not seen Peng crossing after he came to a complete stop at the crosswalk which had a "right turn on red after stop" sign. He said he was going "one or two miles per hour" when Peng collided with his passenger side door. Ross stated he got out to help her and offered to take her to the hospital.

Operator of vehicle 1 states she was traveling westbound on Washington St when vehicle 2 turned quickly in front of her giving her no time to stop and they crashed. As a result of the crash her car rolled backwards into the opposite curb. Operator of vehicle 2 states he was traveling eastbound on Washington St and was waiting to turn left onto Concord St. The traffic light on Washington turned yellow and he went to turn when vehicle 1 came at him at a high rate of speed and hit him head on causing his vehicle to spin around and strike vehicle 3 on Concord St head on. Operator of vehicle 3 stated he was stopped on Concord St at the light to Washington St when vehicle 1 struck vehicle 2 and pushed him into his vehicle. After the accident I spoke to a witness who stated he had video of the incident. Jose Buenrrostro works at 2310 Washington St and was able to show me the footage. In the footage you can see vehicle 1 not slow down for the yellow light on Washington which turns red before she reaches the intersection. She then attempts to swerve around vehicle 2 but was unable to causing the collision. All three vehicles were towed by todys. The operator of vehicle 1 was transported to Brighams Hospital by Cataldo for injuries. Both other operators signed patient refusals with the medics.

Operator of MV1 states she was traveling eastbound on Washington Street when she entered the intersection where Rt. 95 on/off ramps and Quinobequin Rd meet. Operator of MV1 states she had the green signal when MV2, described as a dark colored pick up truck, ran the red light traveling northbound from the off ramp of Rt. 95 southbound/Quinobequin Rd. and crashed into MV1 head on. Operator of MV1 states MV2 fled the scene traveling northbound in the Do Not Enter lane traveling towards Grove St. Operator of MV1 states MV2 sustained front end damage, while MV1 sustained damage to the right front fender and was towed from the scene. Operator of MV1 sustained a wrist injury and was transported to Newton Wellesley Hospital. While investigating the scene, the license plate (MA Reg: 1YF845) from MV2 was left behind from the crash. The registration plate came back on a gray 2017 Honda Ridgleline pick up truck, which matches the description of the vehicle involved in the crash. The vehicle is registered to Andrew Wiggin out of 32 Marlborough Rd Salem, MA. Two witnesses, Lisa Duncan and Jennifer McQuillan, witnessed the crash. Ms. McQuillan states she was behind MV1 when she observed MV2 run the red light, crash into MV1 head on and fled the scene northbound in the Do Not Enter lane. Ms. Duncan states she was a few car lengths behind MV1 traveling on the right hand side of MV1. Ms. Duncan also states she observed MV2 run the red light, crash into MV1 head on and fled the scene northbound in the Do Not Enter lane. Both Ms. McQuillan and Ms. Duncan believe a male was operating the vehicle.

Vehicle #1 was traveling west on Newtonville Ave. As Vehicle #1 passed Lewis Terrace Vehicle #2's front driver side end collided with the side of Vehicle #1. Vehicle #2 was traveling south on Lewis Terrace. Vehicle #2 failed to stop at the Stop Sign at Lewis Terrance and Newtonville Ave and attempted to cross the west bound lane of Newtonville ave to make a left turn to go east on Newtonville Ave. Vehicle #2 failed to yield to Vehicle #1 who had the right of way. The driver of Vehicle #2 was cited with MGL 89/9 Stop Sign Violation and MGL 89/8 Fail to Yield to Oncoming Traffic (Citation #T2012618). The Driver Of Vehicle #2 was not injured. The Driver of Vehicle #1 suffered minor non life threatening injuries and was transported to Newton Wellesley Hospital. Vehicle #1 suffered heavy damage to it's passenger side and front end. Vehicle #2 suffered minor damage to it's front end driver side. Both Vehicles were towed by Todys. A towed MV form was completed and filed for both vehicles Photos of the damaged property at #3 Lewis Ter. was photographed and turned into the IT bureau.

On 03/18/20 at approximately 13:01 I responded to the intersection of Winchester St and Heatherland Rd. for a two vehicle crash. Upon arrival I spoke with Newton Police Detail Officer Schaefer who was involved in the accident. Officer Schaefer was operating his own vehicle (Mass Reg#1JP123 Blue 2001 Cadillac Deville) and headed back to a paid Feeney Brothers detail. Officer Schaefer stated he was traveling due South down Winchester St. approximately 30mph when a light colored vehicle (later determined to be Mass Reg#9FG942- Grey 2010 Toyota Prius operated by Rosana Cristina Brasileiro) pulled out of the one-way Heatherland Rd, the wrong way. Officer Schaefer stated the Prius struck him on his drivers side front and rear doors causing the vehicle to crash into the curb and causing his side air bag to deploy. Officer Schaefer was wearing his seatbelt but struck his head off the interior of his vehicle. I attempted to speak with the operator of the other vehicle Rosana Brasileiro but she only spoke Portuguese. Newton Fire Department Engine #7 was also on scene and Fire Fighter Ian Moniz stated he spoke a different dialect but was able to communicate with her. Fire Fighter Moniz translated for me and I was able to ascertain that Brasileiro stated she was driving due West on Heatherland Rd(wrong way) when she didn't see the other vehicle and made contact with it. I asked Brasileiro for a License and she handed me a Brazilian Passport and stated she didn't have a license. She stated she was just visiting from Brazil and that she was operating her bosses vehicle. Brasileiro was handed Mass. Criminal Citation #T2080843 for Ch90/ Sec10- Unlicensed Operation, Ch90/ Sec 16-Operating on a one way the wrong way and Ch 89/ Sec 8-Failure to yield to the right of way. Both vehicles were towed from the scene while Officer Schaefer went with the Medics to Newton-Wellesley Hospital to get checked out. Sgt. McLean and Traffic Officer Wilson (N523) were also on scene and Officer Wilson took pictures. Sgt. McLean traveled to N.W.H. to check on Officer Schaefer. All roads mentioned/traveled on in this report are public ways in the City of Newton as they are serviced and maintained by this city.

On Saturday, August 8th 2020, at approximately 6:13am, I, Officer Brooks, responded to a 2 car MVA at the intersection of Cherry street and Webster street. Upon arrival I spoke with the operator of MV1(MA REG 1XKP51), who stated he was traveling southbound on Cherry streeet, and failed to stop for the red light at the intersection of Webster street. The front end of his vehicle struck MV2 (MA REG 443LP3) on the passenger side front end. MV1 had damage to the front end and was towed by Tody's. I spoke with the operator of MV2 who stated she was traveling westbound on Webster street and had the green light. As she proceeded through the intersection she was struck by MV1. MV2 had heavy damage to the front passenger side, and had airbag deployment. The operator complained of chest pain and was transported to Newton Wellesley hospital. Her vehicle was towed by Tody's. Tow inventory forms were filed.

Newton Police were advised by a local towing service of a motor vehicle collision at the intersection of Washington St. and Prospect St. Upon arrival, Officers observed MV#1 to have heavy front end damage with air bag deployment while MV#2 had heavy driver side front and rear door damage, also with air bag deployment. The operator of Mv#1 stated she was traveling northbound on Washington St. through the intersection of Prospect St. with a green light, when Mv#2 traveled westbound on Prospect St. through the intersection. MV#1 collided into the driver side of MV#2. MV#2 stated the traffic light was green he was traveling westbound on Prospect St. through the intersection of Washington St. when he was struck by Mv#1. A witness informed police, he was behind MV #1 (northbound) on Washington St. and observed the traffic light to be green. (see witness statement). The operator of MV#1 refused medical attention, while the operator of MV#2 was transported to the hospital. Both vehicle were towed away from the scene. A witness, whom requested to remain annoymous, stated they were traveling behind north bound behind MV#1 and observed MV#1 and several other vehicles traveling northbound through the intersection. The witness stated that vehicles were traveling southbound through the intersection as well, leading officers to believe MV#1 was traveling lawfully through a green light when the collision occured with MV#2.

On Friday 7/9/2021 I, Officer Cunning went to an MVA at the intersection of Union st and Langley Rd. Mv 1 was traveling eastbound on Union St a public way in the city of Newton and rolled through a stop sign. Then MV 1 struck MV 2 on the left side as MV 2 was traveling north bound on Langley rd. I observed heavy damage on the front end of MV1 and heavy damage to the left side of MV 2. The left side airbags on MV 2 deployed. Tody's towed both vehicles. The operator of MV 2 was transported to Newton Wellesley hospital with minor injuries.

The operator of Vehicle #1, Ms. Lea Roy, stated that as she was travelling Northbound on Adams St she observed Vehicle #2 stopped with it's left indicator light on. Vehicle #2 then proceeded to take the left turn onto Middle St prior to the collision with Vehicle #1. Roy complained of pain to her wrist area, but declined transport. After the collision, the operator of Vehicle #2, Mr James MacCormack exited his vehicle and began to walk over to Vehicle #1. He then fell over and struck his head on the pavement. As he was being treated by the medics, he stated that he suffers from diabetes, and hasn't checked his blood sugar in a while. The medics confirmed that his levels were off. He was then transported to Newton-Wellesley Hospital via Medic-5.

Narrative:
Centre Street and Church Street are public ways in the City of Newton. The OP. of MV#1 states he was travelling southbound on Centre Street, proceeding through a green light when a vehicle suddenly came out of a side street and collided with him. That vehicle struck the curb and continued down Centre Street without stopping. The OP. of MV#2 states he isn't from around here and was trying to get home to Worcester. He was driving down an unknown roadway when a vehicle suddenly came into his lane and collided with him. There weren't any shoulders on the roadway so he couldn't pull over right away and kept driving until he found a safe area. Witness#1 (Mr. TING) stated he was driving behind MV#1 and they had the green light on Centre Street. MV#2 came out of Church Street and collided into MV#1, then crashed into the curb and continued driving down Centre Street without stopping. Witness#2 (Mr. O'CONNELL) stated he was at the front porch of 150 Church Street when he heard two loud crashes. He observed a motor vehicle accident and a black vehicle kept driving without stopping and the silver vehicle was stopped at the intersection. Witness#3 (Mr. SPENSIERI) stated he was at the front porch of 150 Church Street when he heard two loud crashes before looking over. He observed a motor vehicle accident where another vehicle kept driving away from the scene. He ran after the vehicle that wasn't stopping (MV#2) and was able to follow along because MV#2 was going slowly with blown tires. He followed MV#2 down Centre Street towards Washington St. MV#2 took a right at the intersection and continued down Washington Street before making another right turn onto Park Street. MV#2 finally stopped at Elmwood Street by Park Street where Mr. SPENSIERI phoned it in. The OP. of MV#1 signed a patient refusal with medics. The OP. of MV#2 complained to Sgt. Walton of a head injury and was transported to St. Elizabeth's Hospital. There was a MBTA bus sign knocked over at the accident location. There are multiple parking spots on Centre Street and on Vernon Street that the OP. of MV#2 could have pulled over to be safely out of traffic but he did not. He drove for approx. 0.3 miles on two flat tires before pulling over at Elmwood Street. The OP. of MV#2 was advised that he will be receiving a citation in the mail for his motor vehicle violations.

Narrative:
Bicyclist Winter Douglass stated that they were riding their bike east bound in the bike lane on Beacon St near Union St on 11/01/2021 at 18:20 hours. Douglass stated that an SUV ( MA reg 2ZVE39 2019 Chevrolet Tahoe ) that had taken a prohibited left turn from Union St onto Beacon St was blocking the bike lane. Douglass stated that they attempted to pass the SUV on the right but could not. Douglass stated that they crashed into the rear driver's side of the SUV. Douglass was wearing a helmet at the time of the crash. Douglass's bike was equipped with lights on the front and back. Douglass was transported to Newton Wellesley Hospital via EMS for a left shoulder injury. Susan Hynds the operator of the SUV stated that they turned left from Union St on to Beacon St and were stopped in traffic when the crash occurred. Hynds stated that they did not see the posted sign prohibiting left turns from Union St onto Beacon St. Hynds stated that she and her son, Sean Hynds ( DOB 07/12/2010 ) who was a passenger in the SUV were not injured. Hynds's SUV had a dent in the rear driver's side and it did not require a tow. I took photo's of the SUV and the posted no left turn sign and turned the disk over to the NPD's IT Bureau. Douglass's bike had no visible damage and it was transported to the station and secured at Support Services. Narrative:
On Tuesday, November 23, 2021, while assigned to Traffic unit N525, I was asked to investigate a past hit and run crash involving a City of Newton School Traffic Supervisor. Mr. Mark Dumoulin reported to the Newton Police Traffic Bureau that sometime during his assigned school crossing post on the morning of November 23, 2021, a passing vehicle made contact with his left leg while he was crossing children on Ward Street. I responded to Newton Wellesley Hospital to speak with Mr. Dumoulin. Mr. Dumoulin stated this morning, he was at his assigned school crossing post of Ward Street and Waverly Avenue in his official capacity as a City of Newton School Traffic Supervisor. Mr. Dumoulin stated he was wearing his department issued yellow reflective jacket and his department issued yellow hi-vis gloves. Mr. Dumoulin stated he also had his department issued stop sign with him while performing his duties. Mr. Dumoulin stated at some point during his post (0745-0830 hours), he was attempting to cross two children on Ward Street (S to N). Mr. Dumoulin stated he entered the crosswalk of Ward Street on the NE corner of Ward Street at Waverly Avenue. Mr. Dumoulin stated the pedestrian signal was activated at this time and the intersection traffic signals were all red. Mr. Dumoulin stated while entering the crosswalk, he observed a black Honda sedan traveling on Stuart Road (E) towards the intersection of Ward Street and Waverly Avenue. Mr. Dumoulin stated the license plate had 3 characters on it and believes the first letter may have been "E". Mr. Dumoulin stated the operator was a heavy set female with blonde hair in her 30's or 40's. Mr. Dumoulin stated the operator appeared to speed up towards his location to beat the light turning red. Mr. Dumoulin stated the vehicle then continued through the intersection onto Hammond Street (E), but it did not keep right with the normal flow of traffic and entered the Westbound lane of Hammond Street. Mr. Dumoulin stated he attempted to get the operators attention to stop her from continuing in the wrong lane of Hammond Street. Mr. Dumoulin stated at this time, the vehicle made contact with his left knee area and continued traveling down Hammond Street with out stopping. Mr. Dumoulin stated the children he was crossing were not near the vehicle and they did not see the contact. I asked Mr. Dumoulin if there were any other witnesses in the area. Mr. Dumoulin stated he does not believe there were. Mr. Dumoulin reported this collision to the Newton Police Traffic Bureau at approximately 1030 hours. I asked Mr. Dumoulin why he did not contact us when the incident first took place. Mr. Dumoulin stated he thought his injury was superficial and that he knew we were busy. At this time, Mr. Dumoulin is reporting a left knee injury as a result of the crash. Mr. Dumoulin stated vehicles travel on the wrong side of the medium on Hammond Street all the time and estimates it happens 3 times every school post. Mr. Dumoulin also stated the keep right sign on Hammond Street near the crash site needs to be addressed because some of the letters are peeling. I canvassed the area where the crash occurred for the fleeing vehicle with a negative result. I also covered the afternoon school post (1450-1535 hours) to ask passing students if they witnessed the crash with a negative result. I observed the "Keep Right" sign Mr. Dumoulin made mention of at this location. The sign appears to be in working condition and visible from Stuart Road. I took photos of the intersection where the crash happened and submitted them to the IT Bureau. On November 24, 2021, I covered Mr. Duboulin's school crossing post again for the morning post. While canvassing the area I observed a dark colored Mercedez sedan with a 3 character registration parked on Exmoor Street that resembled the vehicle Mr. Duboulin described as the fleeing vehicle. I attempted to contact the registered owner of this vehicle, Ms. Judtich Zukor. Ms. Zukor stated she could not speak at this moment and asked me to call her back. I will follow up with Ms. Zukor.

Narrative:
The Operator of Vehicle #1 stated that she was distracted by something that she saw on the sidewalk, and must have drifted to the right before colliding with Vehicle #2. Vehicle #2 was parked and unoccupied; it collided with Vehicle #3 as a result of the Collision from Vehicle #1. Vehicle #3 was parked and unoccupied; it collided with Vehicle #4 as a result of these prior collisions. Vehicle #4 was also parked and unoccupied. The operator of Vehicle #1 was transported to Newton-Wellesley Hospital, and her vehicle towed by Tody's Towing Co. The 3 other vehicles had tows arranged by their owners.
