While MV#2 was parked, the vehicle was sideswiped by MV#1 which was travelling S/B on Cherry St. Road conditions were slippery and it was snowing at the time of the crash. MV#2 suffered damage to Drivers side door and drivers front quarter panel. MV#1 suffered damage to front passenger side quarter panel. No injuries reported.

Leary was crossing Needham St southbound at the corner of the property between Stop and Shop and Jiffy Lube. She states that she was 1/4 of the way across the street when she was struck by MV1. The left side of her body(arm, hip) struck the right side of the MV. She fell to the ground. MV1 states that he was pulling out of the Stop and Shop driveway to turn right onto Needham St. He saw the pedestrian next to the snow bank on the right side of the driveway as he approached the end of the driveway. When he got to the end he did not see her anymore and assumed she had continued walking west on Needham St. He then turned right and felt the collision and then he saw her. He immediately pulled into the Jiffy Lube lot to check on her. I took pictures of the scene and the MV. They were TOT IT. After further investigation, on 2/7 the operator was cited for 90/14 failure to stop for pedestrian. I spoke with Kenneth via phone and advised him of the update. The citation was mailed to him. I am charging for 90/14 failure to stop for a pedestrian. On Wednesday February 6, the operator information of this report was ammended to show the correct operator information.

At approximately 0550HRs on Thursday, February 07, 2019, I was dispatched to the intersection of Washington St. at Centre St. for a report of a two car MVA. Upon arrival, I spoke with the operator of MV1 (CT Pass: 981YJX) who stated she was in the rotary and was attempting to switch lanes by the concrete island to enter I90 Eastbound. As she did so, she collided with MV2. I then spoke with the operator of MV2 (MA Con: S15313) who stated that he was attempting to change lanes and enter Centre St. Northbound by the concrete island. As he attempted to do so, he yielded at the sign on the concrete island and when he did not see any oncoming traffic, he attempted to proceed and collided with MV1. I observed damage to the left fender, left front bumper/hood area and left front wheel area of MV1 and damage to the right door of MV2. MV2 was removed from the scene by Todys Services (NPD Towed MV Form completed/filled). Both parties stated they were uninjured, I assisted in exchanging information and cleared without incident.

The OP of MV#1 states while travelling northbound on Moulton Street, he observed MV#2 sanding the intersection of Cornell Street so he stopped. MV#2 then started reversing in his direction and when MV#2 didn't stop, the OP. of MV#1 then honked his horn but they still collided. The OP. of MV#2 states he was sanding Cornell Street and was doing a U-turn at the intersection of Moulton Street. He didn't observe MV#1 being on Moulton Street at this time so continued sanding the area. When he reversed, that's when his rear tire collided with MV#1. Pictures were taken by me.

Operator of MV1 states he was traveling westbound on Crafts Street attempting to take a left on Waltham Street when a large SUV in front of him stopped in the middle of the intersection because of oncoming traffic from Waltham Street eastbound. Operator of MV1 states at the time he had a green traffic light, but while in the middle of the intersection it turned red. At that time, the SUV and MV1 attempted to turn left onto Waltham Street when MV2 struck MV1. MV1 sustained damage to the front fender and was towed. Operator of MV2 states he was traveling eastbound on Waltham Street approaching Crafts Street when he saw a large SUV in the middle of the intersection. Operator of MV2 states when he entered the intersection, the SUV took a left onto Crafts Street and when MV2 proceeded into the intersection MV1 appeared from behind the SUV and MV2 struck MV1. Operator of MV2 states MV1 was completely obstructed by the large SUV and didn't know he was in the middle of the intersection. MV2 sustained damage to the left front corner panel and was towed.

On 04/25/2019, at 1753 hrs, I responded to 193 Adams St for a two car MVA. Upon arrival, I observed MV1 to have front passenger side damage and MV2 to have rear passenger side damage. The operator of MV1 stated she was attempting to take a left turn out of her driveway and had difficulty seeing due to parked cars. She then collided with MV2 who was traveling northbound on Adams St. The operator of MV2 stated she was traveling northbound on Adams St when MV1 pulled out of a driveway and they collided. No injuries were reported and both vehicles were driven from the scene.

MV1 was traveling east on Madison Ave from Walnut St approaching #80. There were 2 landscape vehicles parked on left side of the street. The operator, Sarah Ruth Loper-Sengupta, stated that she never saw the pedestrian that was in the roadway (N) due to solar glare. Loper-Sengupta stated she heard a thump coming from the passenger side of her vehicle. Loper-Sengupta stated she looked over to her right and saw a trash barrel in the air. Loper-Sengupta stated she then saw a male party lying on the roadway on her passenger side. Immediately after striking the pedestrian, Loper-Sengupta, stopped to check on him. There was a golden doodle dog in the back seat of the MV at the time of the accident, unrestrained. There were no obstructions hanging from the mirror or windshield area. The front drivers visor was down. There was no airbag deployment. I observed significat damage to the front passenger side windshield area. There was also damage to the front passenger side hood area. See Sgt. Lee update for victim statement Numerous pictures were taken by Ofc. Gaudet of the MV and scene. They were TOT IT. The MV was towed by Tody's due to its shattered windshield. No citation will be issued to Loper-Sengupta. The solar glare caused her to not able to see the pedestrian. The Sgt. and I dont believe the pedestrian looked before crossing with the barell on his left shoulder because he would have been able to see the MV coming as this is straight away section of Madison Ave. It should be noted, that when I arrived on scene I immediately noticed the seolar glare as I walked east on Madison Ave.

The operator of MV#1 stated that she was traveling east on Adams Ave in the area of #125 when all of a sudden a large piece of construction equipment entered the roadway from #125 Adams Ave and struck the roof/windshield of her MV. The operator of MV#2 stated that he was attempting to load the Lull onto the flatbed parked on the street by exiting the construction site located at #125 Adams Ave. According to operator #2, as he was entering the roadway MV#1 attempted to go underneath the fork at the front of the Lull and made contact with the fork. Operator #2 may have had his view obstructed by his large truck parked to the left of the exit when he was entering the roadway.

MV1 operator states he was driving Eastbound on Washington St. passing the Rt. 128 off ramps when MV2 collided with his vehicle. No injuries reported on scene. MV1 was able to drive his vehicle to Belmore Pk. where he will wait for AAA. I observed damage to the right side and rear right side of the vehicle. MV2 operator states he come up to the stop sign at the Rt. 128 off ramp, stopped and then proceeded when clear. As MV2 operator was crossing over the lanes to take a left hand turn to go Westbound on Washington St., he did not see MV1 in lane 1. At this point, MV2 collided with MV1. On 05/20/2018 at 17:30 hours I was notified by Captain Anastasia of a motor vehicle crash involving car, and an electric scooter. Captain Anastaisa stated that the operator of the scooter was a 12 year old male who had sustained unknown injures and had been transported to Boston Children's Hospital via ambulance. Upon my arrival at the crash scene I made the following observations, Putnam and Winthrop streets are public ways in the city of Newton that are two way single lane non divided roadways. The roadway surface was wet from rain earlier in the day. Putnam St runs north to south and Winthrop St runs east to west. There is a stop sign at the end of Winthrop St at Putnam St. I observed MA reg 54MN41 a 2005 Buick LeSabre stopped in the south bound lane of Putnam St approximately 1/2 of the way into the intersection with Winthrop St. I observed approximately three feet of skid marks from the Buick's rear tires. I also observed at the crash scene a Segway Ninebot electric scooter that was on the ground near the Buick(cid:31)s rear passenger side door with its handlebars detached. I spoke with the operator of the Buick, Robert DeSouza who stated that on 05/20/2019 at approximately 16:50 hours he was driving south bound on Putnam St at approximately 25 miles per hour. DeSouza stated that he saw a man, later identified as Jose Estabil standing on the east sidewalk of Putnam St facing Winthrop St who was yelling and waving his arms at something on Winthrop St. DeSouza stated he looked to his right towards Winthrop St and his view was obstructed by a row of hedges. DeSouza stated that as he was looking to his right he began to apply his vehicle's brakes when he saw someone coming at me ". DeSouza stated that he heard a crashing noise from the rear passenger's side of his vehicle. DeSouza stated that he stopped his vehicle after the crash and saw a boy lying on the ground at the rear passenger side of his vehicle. DeSouza stated that he was not injured in the crash. DeSouza's vehicle had minor damage to its rear passenger side door, and did not require a tow. Witness Charlotte Hart stated that at approximately 16:30 hours on 05/19/2019 she was diving south bound on Winthrop St. Hart stated that she had to stop short on Putnam St to avoid a crash " with a boy on a scooter that was racing around on Winthrop St. " Hart sated that she told the boy on the scooter that he should not be ridding the scooter in the street. Witness Jose Estabil stated that he was walking on the east side walk of Putnam St across from Winthrop St. Estabil stated that he saw a boy an electric scooter on Winthrop St approaching Putnam St. Estabil stated that he saw DeSouza's vehicle driving south bound on Putnam St approaching Winthrop St. Estabil stated that the boy on the scooter made no effort to stop, slow or avoid the crash with DeSouza's vehicle. Witness Laurel Fransworth stated that she was driving north on Putnam St approaching Winthrop St when she saw the crash occur. Farnsworth stated a boy on an electric scooter on Winthrop St crash into the rear passenger side door of DeSouza's vehicle. Fransworth stated that she believed that the row of hedges at the end of Winthrop St obstructed DeSouz's vehicle. Rowan Delaney ( 02/27/2007 ) stated that on 05/20/2019 at approximately 16:00 hours he and his fellow F.A. Day Middle School student Thiaugo Rubino ( 03/04/2007 ) were having a race around the area of Winthrop St, Putnam St and Shawn St. Delaney stated that he was on foot running on sidewalks while Rubino was on an electric Ninebot Segway scooter. Delaney stated that Rubion was not wearing a helmet or any other safety equipment. I spoke with Delaney(cid:31)s father Royston Delaney, who stated that the scooter was his and that he was unaware that Rubino was using his scooter, and that he has never seen him ridding it before. Royston Delaney stated that the scooter is battery powered and has a maximum speed of 19 MPH. Royston Delaney stated that his son told him that only Rubino was ridding the scooter. Rubino was transported via ambulance to Boston Children(cid:31)s Hospital. On 05/20/2019 Rubino underwent surgery for a brain bleed. I spoke with Rubion(cid:31)s mother Sonia Rubino who stated that as of 05/21/2019 her son was under observation and in addition to the brain surgery her son had a broken arm, broken leg and a broken back. Based upon witnessed statements and my observations of the crash area I believe that Rubion was at fault for the cause of this crash. Myself and Officer Helms took photos of the crash area and the disks were turned over to the NPD(cid:31)s IT bureau for downloading.

MV#2 was attempting to turn left onto the north side of Centre St from Pleasant St. Operator of MV#1 stated that as she was traveling south, a car pulled out from Pleasant St. Operator of MV#1 stated that there was a large truck in the right lane blocking her view, and she did not see MV#2 pull onto Centre St. MV#1 crashed into MV#2. Operator of MV#2 stated that a truck had stopped in the right lane of Centre St, allowing her to take a left off of Pleasant St. Operator of MV#2 stated that as she was attempting to turn left onto Centre St a MV#1 was in the left lane and crashed into her. Both vehicles were driven off scene. One passenger of MV#2 stubbed her toe during the crash, and was cleared with a patient refusal.

On 07/25/19 at 10:28 hours, I responded to Centre Street and Boylston St off ramp (WB) for a two car motor vehicle accident with no injuries reported. Operator of MV1 states he was traveling straight ahead on Centre Street (NB) when he observed MV2 inching out of the Boylston Street off ramp. He was not able to stop in time and the two vehicles struck one another. I observed heavy damage to the passenger front bumper and tire area and the vehicle was towed by Tody's. Operator of MV2 states he was exiting the Boylston Street off ramp (WB) and was going to attempt a left turn on to Centre Street after stopping for the stop sign. He then slowly pulled out into the intersection due to National Grid trucks blocking his view and that is when he came in contact with MV1 on his driver's side front bumper. I observed moderate damage to the entire front bumper but the vehicle was still driveable. I did observe National Grid trucks parked at the intersection setting up to do construction work. One truck was parked on the left side of the off ramp and the other was parked on the side where the crash occurred. This did obstruct the operator of MV2's view when pulling out.

At approximately 1640 HRs on Wednesday, July, 31, 2019, I responded to the intersection of Centre St. at Hollis St. for a two vehicle crash. The operator of motor vehicle 1 stated she was traveling eastbound on Hollis St. while motor vehicle 2 was attempting to enter Centre St. southbound after stopping at the stop sign controlling the intersection. At this point, motor vehicle 2 reversed and struck motor vehicle 1 at the stop sign. The operator of motor vehicle 2 stated that he was entering the intersection in an attempt to travel southbound on Centre St. when traffic already southbound on Centre St. was flowing. In an attempt not to block northbound traffic, he reversed and accidentally struck motor vehicle 1. motor vehicle 1 sustained damage to its left fender and driver's side door areas. motor vehicle 2 sustained damage to its right rear bumper area and had preexisting scrapes to its right rear quarter and sliding door.. Neither party were injured and both vehicles left under their own power. I cleared without incident.

On Friday, August 16th 2019, at approximately 6:40pm, I, Officer Brooks, responded to Dunstan street at Washington street for a MVA pedestrian. Upon my arrival I spoke with the operator of MV1(294NZ5), who stated she was traveling southbound on Dunstan street and was attempting to pull out onto Washington street, when she made contact with a bicyclist traveling westbound on Washington street. The operator stated she never saw the bicyclist, and that she was traveling less than a mile per hour when she made contact. It should be noted that the sun was setting and the operators vision would have been impaired by the sun at the location of the accident. MV1 had a minor scuff mark on the front bumper. I spoke with the bicyclist who stated he was traveling westbound on Washington street and was crossing over Dunstan street when he was struck by MV1. He stated he was knocked off the bike, scraped his arm, and had minor back pain. The mirror on the right side of his bike was broken. The bicyclist was wearing greenish gray clothing, and was not wearing protective equipment such as a helmet. The bicyclist signed a refusal from Cataldo and was given a copy of the report number. Pictures were taken of the scene and all parties were sent on their way.

Valentine Baron stated on she was bicycling in the marked crosswalk at Center Ave at Center St from the north side walk to the south side walk. Baron stated that she activated the crosswalk light and had entered the crosswalk when the light was activated. Baron stated that while she was in the marked crosswalk vehicle one began to make a right turn on red . Baron stated that vehicle one struck her on her left side , knocking her off her bike . Baron stated that she had a helmutt but was not wearing it the time of the crash. Baron stated that she did not lose consciousness as result of the crash. Baron was transported via EMS to Mt Auburn Hospital for treatment for possible internal injuries. Operator of vehicle one Sara Ross stated that she was stopped at a red light on Center Ave at Center St. Ross stated that she was waiting to turn right on a red light and look to her left and then her right and drove forward. Ross stated that she did not see Baron in the marked crosswalk. Charles Hannum was a front seat passenger in vehicle one and stated that he did not see Baron in front of the vehicle until the crash occurred. I took photos of vehicle one and forwarded to the NPD IT bureau. I issued Ross M Citation T21065501 and cited her for a violation of CO 19/74 , failure to use care while turning.

The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave making a left turn onto Morton St when he was struck by MV#2. The operator of MV#1 stated he was unsure of where MV#2 came from. There was a strong solar glare facing westbound on Commonwealth Ave during this accident. MV#1 sustained heavy driver's side front end damages. Both front airbags were deployed. The operator of MV#1 stated he was not injured. The front passenger of MV#1 was not injured and had already left the scene to attend school. The driver's side rear passenger sustained a bleeding nose from this accident. She was unsure what caused her injuries because it happened too quickly. She was evluated by Newton Paramedics and signed a patient refusal of treatment. The initial point of impact for both vehicles was in the middle intersection of Commonwealth Ave at Morton St. There were broken vehicle parts and tire skid marks starting from the point of impact and onto the berm on Commonwealth Ave on the carriage lane side. MV#2's final resting spot was directly accross from 808 Commonwealth Ave. MV#2 sustained heavy front end damages to its driver's side wheel (Broken axle) and flat front right side tire. MV#2's driver's side airbag was deployed. The operator of MV#2 was transported by Newton Paramedics to the Boston Medical Center. I spoke to the operator of MV#2 via telephone and she stated she was travelling westbound on Commonwealth Ave when she was struck by MV#1 and caused her swerve onto the carriage lane berm. The operator of MV#1 is 6.5 months into her pregnancy and sustained neck, back, bruises and left hand injuries. She was transported to Boston Medical Center and was later discharged the same day. She was provided with this accident report number and was advised to contact her insurance provider. I spoke to the witness who was at the intersection of Cedar St and Commonwealth Ave. The witness stated MV#1 was travelling eastbound on Commonwealth Ave and had stopped, attempting to make a left turn onto Morton St. The witness stated she then observed MV#2 travelling westbound on Commonwealth Ave and struck MV#1. The witness stated she believed MV#2 was travelling at a high rate of speed because it continued to travel onto the berm after striking MV#1. I took digital photographs of the damaged berm on Commonwealth Ave. Based on the statements made to me, I issued in hand to the operator of MV#1 citation #T1268357 for violation of c89 s8 failure to yield when turning left. Both vehicles were towed by "Tody's Towing".

On Wednesday, October 16, 2019, I, Patrolman Hatfield, was assigned to marked cruiser N 495 to Division 5 for the 1530 to 2330 hour shift. At approximately 1742 hours, FTO Guarino and I were dispatched to Commonwealth Ave at Cedar St for a motor vehicle accident with minor injuries. FTO Guarino and I arrived on scene and noticed two MVs with damage. I spoke with the RO/operator of MV2, Janet Haines (Janet). Janet stated she was not suffering from injuries. Janet said she was operating southbound on Cedar St crossing Commonwealth Ave when she had a collision with MV2. Janet stated prior to crossing Commonwealth Ave, an unknown MV stopped at the intersection to allow her to cross. Janet then stated she proceeded accross when she crashed into MV1, who drove around the stopped MV. Janet said her brakes did not work and she continued to drive forward and ended on top of the curb of Commonwealth Ave facing eastbound. MV2 suffered damage to the whole front bumper and wheel wells. FTO Guarino spoke with the operator of MV1, Madeleine Richard (Madeleine). Be advised both front and side driver airbags were deployed, and the front driver's side door was unable to be opened. I then spoke with Madeleine. Madeleine stated she was operating eastbound on Commonwealth Ave before being involved in the collision. Madeleine stated she drove around the MV infront of her that was attempting to take a left from Commonwealth Ave onto Cedar St, and she did not see MV2 prior to the accident. MV1 was struck on the driver's side by MV2, and ended facing westbound in the middle of Commonwealth Ave. EMTs and fire arrived on scene. Janet stated she did not need to be transported. Madeleine was transported to Newton Wellesley Hospital for an evaluation. Both MVs were towed by Todys.

On Wednesday October 23, 2019, I, Patrolman Hatfield, was assigned to marked cruiser N495 in full uniform to Division 5 for the 1530 to 2330 hour shift. At approximately 2040 hours, myself and Officer Selig were traveling northbound on Centre St (a public way) in Newton Center. We were behind MA reg 9KT873 (2004 Toyota Corolla grey). We observed the motor vehicle drive around a pedestrian in the crosswalk who was attempting to cross at Centre St and Pelham St. I activated my emergency blue lights and attempted to conduct a motor vehicle stop of the Toyota Corolla. The operator continued to drive northbound on Centre St before pulling over just before the crosswalk at Centre St and Pleasant St. The operator then continued driving forward and struck a pedestrian crossing in the crosswalk Officer Selig and I saw the pedestrian being struck before landing on top of the Toyota Corolla hood, and falling forward onto the street. Officer Selig and I spoke with the two parties involved. I spoke with the victim, who was identified as Vivian Rubinstein (Vivian). Vivian crawled towards the sidewalk of Centre St while clutching her lower back. Initially Vivian denied being injured and refused medical aid. The Toyota Corolla operator was identified as the register owner Elise Ruben (Elise). Elise stated to Officer Selig that her front windshield was foggy and when she noticed the blue lights, she attempted to pull over. Elise stated she did not see the pedestrian in the crosswalk. Elise appeared very upset and refused medical treatment. Officer Selig was able to speak to two witnesses of the accident. The witnesses were identified as Stephan Aparicio (Stephan) and Mikaela Rice (Mikaela). Stephan said he was behind the victim in the same crosswalk. Stephan stated he saw the Toyota stop in front of the crosswalk and then drove forward hitting the female. Mikaela stated she was parked in her motor vehicle on Centre St facing northbound (facing the crosswalk). Mikaela stated she saw the female walking halfway across the crosswalk and the Toyota continuing drive forward before hitting the female. Officer Siegel arrived on scene and took four photographs which was submitted with this report. Vivian was transported by Newton Medics to Faulkner Hospital for an evaluation. I issued written warning citation number T2079786 to Elise for failure to yield to a pedestrian in crosswalk (Centre St at Pleasant St). I also issued Elise citation number T2079787 for failure to yield to a pedestrian in crosswalk (Centre St at Pelham St). The vehicle was driven from the scene.

OPERATOR OF MV 1 WAS TRAVELING WEST ON WASHINGTON STREET APPROACHING THE RED LIGHT AT CHURCH STREET WHEN HIS VEHICLE WAS STRUCK BY MV 2 WHO WAS TURNING LEFT OUT OF HOVEY STREET ONTO WASHINGTON STREET (EASTBOUND) . MV 1 HAD FRONT END DAMAGE BY THE RIGHT FROM TIRE, NO INJURIES WERE REPORTED AND MV WAS TOWED FROM THE SCENE BY TODYS. OPERATOR OF MV 2 STATED THAT HE WAS ON HOVEY STREET BY WASHINGTON ST, STOPPED AT THE STOP SIGN. A TRUCK WAS IN THE CLOSEST TRAVEL LANE TO HIM AND WAIVED HIM ON TO TURN LEFT. AS MV 2 TURNED LEFT HE COULD NOT SEE MV 1 TRAVELING IN THE OUTER MOST LANE, AND HIS VEHICLE STRUCK MV 1 ON THE PASSENGER FRONT SIDE. MV 2 HAD MODERATE FRONT END DAMAGE AND NO INJURIES REPORTED.

OPMV#1 Was traveling Southbound on Greenough St and came to a stop at the intersection of Washington St and Greenough St. OPMV#1 entered the Westbound lane of Washington St crossing over into the Eastbound lane when she was struck by the OPMV#2. OPMV#1 Stated she was being let go by people stuck in traffic and when she entered the other lane, that is when she was struck by another vehicle. OPMV#2 Was traveling Eastbound on Washington St when a vehicle pulled out in front of him causing him to collide with the OPMV#1. OPMV#2 Stated he was traveling straight on Washington St to get onto the Mass Pike to go to the airport when I vehicle pulled out in front of him and he had no time to stop. MV#2 Was towed by Tody's Towing. OPMV#1 Was transported to NWH by EMS.

On 11/27/19 at 12:43 hours, I responded to the Petsmart located at 215 Needham Street for a motor vehicle bicyclist accident. The weather at the time was approximately 50 degrees, cloudy and the road conditions were dry. No injuries reported. Operator of MV # 1 (Barbara Cooper) states she was facing eastbound on Needham Street waiting in the center turn lanes to take a left turn in to the garage of 215 Needham Street. Next, she states that a large truck going westbound stopped for her and waved her on to turn. She then proceeded in to the garage and parked and that is when she was confronted by the bicyclist saying that she was struck with her vehicle. The operator was unaware of an accident occurring and was not able to visibly see past the truck while turning. I observed a minor scratch to the passenger rear quarter panel area of the vehicle. Operator of the bicyclist (Merissa Brousseau) states she traveling westbound in the bicycle lane on the right side near 215 Needham Street when she observed the large truck mentioned above suddenly stop. She states that she was also visibly blocked from seeing ahead due to the truck. She then kept peddling straight through and struck MV # 1 in the passenger side rear end as it was making its left turn. The operator was evaluated by Cataldo Ambulance but refused medical attention. I observed no apparent damage to the bicycle. Ms. Brousseau was wearing a bicycle helmet, black and red Boston University bicycle suit, and red BU sock covers over her shoes. On Wednesday, 12/4/2019, at approximately 2045 hours, Ofc Helms and I responded to a past hit and run at 27 Parsons St. Upon arrival, I spoke with the owner of MV 1 (Marc Rizza) who stated that he noticed damage to the left rear of his vehicle today when he came back from vacation to pick up his car. MV 1 was parked in the driveway of 27 Parsons St, (the brother in law of the vehicle owner). He further stated that he believed a plow may have struck his car when it was plowing the driveway after or during the last snow storm. Yellow streaks were visibly seen on the left rear bumper consistent with the damage. The owner of MV 1 further stated that he believed the house across the street, 28 Parsons St is the person that usually plows the driveway. I spoke with Jeffrey Timmins who told me that he in fact did plow the driveway of 27 Parsons St after the storm sometime on Tuesday 12/3/2019. Mr. Timmins admitted to being the operator of MV 2 and stated he did not notice if he hit MV 1 while plowing the driveway but he "could have". I advised both parties with an accident report number. No tows required or reported injuries by either party.

At approximately 0500HRs on Tuesday, December 17, 2019, I was dispatched to a 2 vehicle MVA in front of 38 Commonwealth Ave. Upon arrival, I spoke with the operator of MV1 (MA CON: S25585) who stated that he attempted to back up his vehicle (Westbound) and accidently struck MV2 because he was not able to see it in his mirror. Upon speaking to the operator of MV2 (MA CON: S13628), he stated he was traveling Eastbound as MV1 was backing up. He then stopped MV2 and when MV1 did not, he activated his horn which had no effect and MV1 backed into him. I observed MV1 to have no discernable damage. I observed MV2 to have sustained damage to its front bumper, grille, hood, passenger's side fender and both head lamps. MV1 had a single passenger who reports he did not witness event in question, as he was focused on eating at the time. No one involved reports any injuries. I cleared without incident.

The operator of MV#1 stated he was parked at the street bend of 64 Homer St when he was struck from behind by MV#2. MV#1 sustained heavy damages to its driver's side rear bumper area. MV#2 sustained heavy front end damages with both front airbags deployment. The operator of MV#2 sustained a cut lower lip. He was evaluated by Newton Paramedics and signed a patient refusal of treatment. It should be noted when this accident occurred, there was a solar glare obstruction that faced all oncoming eastbound traffic. Tody's Towing took possession of MV#2.

On 02/26/2020 I responded to the area of Eastbourne Rd and Ireland Rd for a motor vehicle crash involving a pedestrian. Eastbourne RD is a public way which runs from Ward St north bound intersecting with Ireland Rd on the far north end. Ireland RD is a public way which runs west to east. Both the east end and the west end of Ireland Rd are dead ends. The crash occured on Ireland Rd's east end near the intersection of Eastbourne Rd. The crash involved Vehicle 1 described as a 2016 Acura SUV color grey bearing MA passenger registration 3RB127. Vehicle 1 had a valid inspection sticker which expires on 08/30/2020. Vehicle 1 was operated by its owner identified as Barbara Ball. Ball has an active MA Operators License (S18727836). The involved pedestrian was identified as Bruce Terrio. Terrio is an employee of Feeney Brothers Utility Services. Feeney Brothers Utility Services is a contractor of National Grid Gas Company. Upon arrival I observed the following: The crash occurred in the daylight hours around 10:30am on Ireland Rd near the intersection of Eastbourne Rd. At the time of the crash the weather conditions were clear, 48 degrees Fahrenheit and overcast skies (weather.gov). The area where the crash occurred was an active construction site for Feeney Brothers Utility Services. The area of the crash is a dead end. The area is accessed from Ward St from either Eastbourne Rd or Coolidge Rd. The entrances of both Eastbourne Rd and Coolidge Rd had construction signs indicating utility work in the area. At the time of the crash, all roadways were open to vehicle traffic. On the eastbound side of Eastbourne Rd just prior to the intersection of Ireland Rd, I observed a large air compressor which was parked in the roadway facing south bound. No traffic cones were located near the air compressor. To the right of the air compressor were thick green bushes. A wooden fence was located to the right side of the green bushes. On the east side of Ireland Rd I observed a large excavated hole on the right side of the roadway. The hole was near the intersection of Eastbourne Rd and was located approximately three feet from the curb. I observed four traffic cones, two on the front side and two on the left side of the hole. Located on the front edge of the hole was a tire print. Vehicle 1 was located on the west end of Ireland Rd. The front passenger tire of Vehicle 1 had fresh dirt located on the sidewalls and the tire tread. Vehicle 1 appeared to have no visible damage. The pedestrian (Terrio) was being treated by EMS and Fire for injuries. Terrio was wearing brown pants and a green reflective safety vest. I spoke with the operator of Vehicle 1 Barbara Ball. Ball stated that she was operating Vehicle 1 northbound on Eastbourne Rd near the intersection of Ireland Rd. Ball stated that she made a right turn onto Ireland Rd and drove on to the edge of the excavated hole. Ball stated that her vehicle then made impact with Terrio who was in the hole. Ball stated that she immediately stopped, backed out of the hole and called 911. Ball stated that prior to making a right turn onto Ireland Rd, there were no traffic cones in the roadway or next to the hole. Ball stated that as she made the right turn onto Ireland Rd she did not observe Terrio or the hole until it was too late. Ball states that she resides at 80 Eastbourne Rd which is the residence located to the immediate right of the crash location. Ball states that she had been recently parking on Ireland Rd due to ongoing construction at her residence. Ball states that she was traveling at a slow speed prior to impact. At the time of the crash there were three other Feeney Brothers employees working at the site. These workers were identified as Steven Gamble, Mike Cabrinma and Gerard McComaskey. At the time of the crash all three workers were located further up Eastbourne Rd near the intersection of Coolidge Rd and did not observe the crash. At the time of the crash, Newton Police Sergeant Glenn Chisholm was working a paid detail for Feeney Brothers. Sergeant Chisholm stated that he did not observe the crash.Terrio was transported by Cataldo EMS for minor injuries. Several photos were taken of the scene. Photos were taken of the hole with traffic cones and without traffic cones. All photo's were submitted to the IT Bureau. After clearing the scene, I responded to St Elizabeth's Hospital in Brighton, MA to interview Terrio. Terrio stated that at the time of the crash he was standing in the hole on Ireland Rd performing gas utility work. Terrio stated that the front passenger side bumper of Vehicle 1 struck him in the right shoulder. Terrio stated that the impact caused him to lose his balance and fall down in the hole. Terrio states that after falling down he was able to climb out of the hole without assistance. Terrio stated that at the time of impact he was wearing a white hard hat which he thinks struck the pavement on the edge of the hole. Terrio states that at the time of the crash he was working alone in the hole. Terrio states that at the time of the crash traffic cones were located near the hole. Terrio stated that as a result of the impact he sustained minor injuries to his right shoulder and back. I then observed Terrio's hard hat which appeared to have a fresh scrape mark on the top. The hard hat also had broken interior straps. Terrio stated that the straps broke when he was struck by Vehicle 1. Photos were taken of the hard hat and were submitted to the IT Bureau. After reviewing all facts of this incident, Vehicle 1 appeared to have been traveling at a slow speed prior to the crash. This was evident due to the lack of tire skid marks prior to the tire print on the edge of the hole. It also appears that Ball's view of the hole and Terrio may have been obstructed by the parked air compressor on Eastbourne Rd. The thick green bushes and a wooden fence to the right of the air compressor also added to the obstructed view. The hole was also located in close proximity to the air compressor which reduced Vehicle 1's reaction time. Jeff Oliver of OSHA was also notified of this incident. On May 16th, 2020 at approximately 10:01 hours while working N491 I responded to #281 Newtonville Ave for a reported single vehicle crash. While en route the reporting party stated the vehicle had left the scene. On my arrival I spoke with the R/P who stated while riding his bike down Newtonville Ave he observed a black colored landscaping pickup truck back out of Pillion Court onto Newtonville Ave and hit a fire hydrant across the Street. Its possible the driver never knew he struck the hydrant. The R/P never saw the driver or the license plate number. Fire department and water department were notified and were responding. Pictures were taken and later submitted to Technology Bureau.

Operator of MV1 stated she was traveling Westbound on Tremont Street and could not see well due to a sun glare. Operator of MV1 further stated when she approached the intersection of Tremont Street at Hibbard Road she struck a pedestrian riding a bicycle. I was able to speak with two witnesses on scene who were friends with Ms. Shokri. Both were approximately 10 feet behind her in the crosswalk before she was struck by MV1. Witness statements are documented in this report. Both parents responded on scene and were entered into this report as non-motorists. Both followed medics to Newton Wellesley Hospital. MV1 sustained moderate front end damage. Pedestrian involved suffered minor injuries and was transported to Newton Wellesley Hospital. Operator of MV1 was evaluated on scene and signed a patient refusal. Due to the bicycle being moved prior to my arrival, pictures were not taken on scene. The bicycle was left off the roadway for her friends to pick up. Ms. Skokri was wearing a pink shirt, black shorts and was riding a black bicycle. Ms. Shokri was not wearing any protective equipment.

Operator of vehicle one Waste Management employee David Echeverria stated on 06/01/2020 at 12:11 hours he was driving MA com reg V36945 a 2009 Expedi trash truck south bound on Johnson Pl to pick up trash barbells. John Pl is a dead end public way in the city of Newton. Waste Management is contracted by the city of Newton to collect trash. Echeverria stated that since John Pl is a dead end it is common practice to back the truck in because making a three point turn at the end of the street would not be feasible. Echeverria stated that MA reg 3SH854 a 2012 Nissan was parked unoccupied facing north bound near 12 Johnson Pl. it should be noted that parking is only permitted on Johnson Pl by city of Newton issued permit only. Echeverria stated that while he backed his truck up the front passenger side bumper made contact with the Nissan's front driver's side quarter panel, front driver's side mirror and front driver's side tire. Owner of vehicle two Mandar Kasture stated he parked his vehicle on Johnson Pl without a parking permit. Since Kasture's vehicle was illegally parked , I issued him a verbal warring. I took photos of both vehicles and turned the disk over to the NPD's IT bureau for downing loading.

On 6/15/2020 at approximately 1540 hours, I responded to 2040 Commonwealth ave for a report of a two car accident. Upon arrival, I spoke with MV 1 who stated that he was reversing his tractor trailer into the shaws loading dock area when he struck MV2 with the backend of his cab. There is minimal damage and no injuries reported. I then spoke MV2 who stated that he was behind MV1 driving westbound on Commonwealth ave. He stopped to let the tractor trailer back into the loading dock. He realized he did not leave the trailer enough room to make the turn and that is when MV1 and MV2 struck. He reports no injuries and minor damage to his vehicle.

Mr. Pleynet states he doesn't have recollection of the accident since he lost consciousness for a brief time after being struck. Mr. Pleynet was transported to Brigham and Women's Hospital where he sustained a fractured left shoulder, abrasion on his right shin and abrasion on his forehead. The bicycle sustained damage to the front wheel and was placed into property and evidence for safekeeping. Pictures of the scene were captured and the SIM card was submitted to the IT bureau.

On Thursday 6/24/21 I responded to a two car motor vehicle accident at the intersection of Lowell Ave and Washington st. Upon my arrival I spoke with the operator of MV 1 (RI REG CONCPT) who stated he was headed westbound on Washington and trying to turn southbound on Lowell Ave. He said his view was obstructed by an eversource detail that was working just before the intersection on Washington st. Pishtikova stated that she walked around the front of Kolaczyk's vehicle and it then turned into her at a very low speed. Pishtikova stated that the front drivers side quarter panel of Kolaczyk's vehicle struck her right side. Pishtikova stated that her right ankle was sore but refused medical treatment. Pishtikova signed a patient refusal form provided to her by Newton Medic Unit Three. I took photos of Kolaczyk's vehicle and the disk was downloaded by the NPD's IT bureau. Kolaczyk's vehicle had no damage and did not require a tow. Kolaczyk stated that she was not injured in the crash. I issued Kolaczyk MA Uniform Citation T2016945 and cited her for a violation of MGL 89/11 , failure to yield to a pedestrian in a crosswalk. Crafts St and Albemarle Rd are public ways in the City of Newton.

V2 MA Reg 389ME3; 2015 Niss/Altima color black. Both operators were out of the vehicles upon my arrival. V2 was having chest, back and neck pain. V2 operator was evaluated by EMS on scene. V2 Operator was transported by ambulance to Newton Wellesley Hospital for further evaluation. V1 operator was uninjured and sign an evaluation waiver with EMS on scene. On scene I spoke with V1 operator who stated he was coming off the (5N off ramp attempting to turn left onto Washington St. (westbound). Both directions of Washington are multi lane. V1 operator described traffic as heavy. V1 operator stated he saw a gap in traffic and proceeded to cross over the eastbound lanes to turn left onto the westbound side. V1 operator stated he never saw V2 traveling in the #1 lane of Washington St (eastbound). V1 had an obstructed view by oncoming traffic in the #2 lane of Washington St. (eastbound) and collided into V2. The crash scene was cleared and both vehicles were towed by Tody's Towing Inc. I did not observe airbag deployment in either vehicle. Once clear of the crash scene I proceeded to NWH to speak with the V2 operator. V2 operator stated she was traveling straight ahead on Washington St (eastbound). V2 operator basically explained never knew what hit her. She never saw V1 and only felt the impact of the collision. V2 operator was waiting to be treated for chest, back and neck pain. V2 was placed in a cervical collar and back board by EMS prior to transport to NWH.

On 10/13/2021 at approx 0803Hrs while assigned to 497 I responded to the area of 349 Auburn St for a report of a two car crash with injury. Upon arrival I observed Ma Reg 1FLA38 disabled in the middle of the road with heavy front passenger side damage, with both front and passenger side airbags deployed. Fallon ambulance was on scene with the operator , John McHugh on the stretcher with a head injury. The Fallon EMT's Alex Honrado and Steve Hough stated they had been parked on Auburn St when McHugh ran into the back of a parked landscape trailer. I spoke with McHugh who stated he could not see out his windshield as he had just left his home on Bourne St and his windshield was not defrosted yet and ran into the trailer. McHugh and Fallon Ambulance personnel said as there had been no damage to the trailer the landscaper had left and nobody got a plate number or company name. McHugh transported to NWH, Todys towed the vehicle. A routine registry check showed McHugh has a suspended license.Ma Uniform Citation T2015320 for criminal application for violation of MGL Chec90 Sec 23 Operating After Suspension mailed to McHugh.

On 10/14/2021 while assigned to marked cruiser N493 at approximately 18:34 hours I, Officer Vazhenin, responded to 307 Auburn Street for a report of 2 car MVA. Upon arrival I spoke to Ms. Jennifer Ann Pope (Driver 1 of MV 1), who stated that when she was taking a left turn to Auburn Street, she followed a car (MV 3) in front of her, because she believed that MV 2 saw her and was letting her to take a left turn. Ms. Pope (Driver 1) said that when she was making that turn, MV 2 started making a turn in the same direction. She stated that she stopped, but MV2 kept driving and crashed into the right front corner of her car (MV1). I then spoke to Ms. Katherine Mary Iodice (Driver 2 of MV2), who stated that she yielded the right of way to MV3 and then started making a turn right after that car. Ms. Iodice stated that she did not see MV1, which was following closely to MV3, and while she was making a turn, she observed MV1, but it was too late to stop and her car (MV2) crashed into MV1. Driver 1 of MV1 was at fault because she failed to yield the right of way to Driver 2 of MV2, which had an obstructed vision and did not see MV1.

Operator of MV1 states he was traveling northbound on Walnut Street and entered the left turn only lane at the intersection of Walnut Street and Beacon Street. Operator of MV1 states while entering the left turn only lane, a pedestrian appeared from behind a car that was in the straight only lane and he struck her at a very low rate of speed with the front end of MV1. Operator of MV1 states the pedestrian said she was fine and kept walking across the street. Operator of MV1 states he pulled into the parking lot of the Walgreen's, got out of his vehicle and attempted to speak with the pedestrian again, which she waved him off stating she was fine and walked away from him. Operator of MV1 at this time left the scene believing the pedestrian was not injured. No damage to MV1. The pedestrian, Elizabeth Kennedy, states she was walking across Walnut Street (not in the crosswalk) when she entered the left turn only lane on Walnut Street and was struck by MV1. Ms. Kennedy states she hit the ground, but got back up and told the operator of MV1 she was fine and he left the scene. Ms. Kennedy complained of knee and elbow pain. Fallon Medics evaluated Ms. Kennedy and she signed a patient refusal, but stated she will have someone transport her to Tuft's medical center.

On 11/08/21 at 14:08 hours while on routine patrol I came across a two car motor vehicle crash in the westbound lane of 667 Commonwealth Avenue. Commonwealth Avenue is a public way owned and maintained by the City of Newton. Both operators were outside of their vehicles at the time and reported no injuries. I observed Vehicle # 1 (Honda Civic MA reg. # 2CWA44) to have heavy front end damage to the bumper and hood area. It was parked back approximately ten feet from Vehicle # 2. Due to the damage and it leaking anti-freeze coolant, Tody's Towing was notified to tow the vehicle away. Vehicle # 2 (Ford EcoSport MA reg. # 2HJV37) was parked in front of Vehicle # 1 and sustained damage to the center and right rear bumper area. It was still driveable. I spoke to the operator of Vehicle # 2 (Maria Bianchi MA D/L # S97972578) and asked her what occurred. She stated that she was traveling westbound on Commonwealth Avenue in her Ford EcoSport (MA # 2HJV37) but was stopped at the time waiting for a vehicle in front of her to make a left turn on to Grafton Street. She was then struck in her rear end by Vehicle # 1. After, I spoke to the operator of Vehicle # 1 (Maria Kuznetsova MA D/L # S48191231) and asked her what occurred. She stated that she was traveling westbound on Commonwealth Avenue in her Honda Civic (MA # 2CWA44) and did not see Vehicle # 2 stopped in traffic due to the sun. She then struck Vehicle # 2 in the rear end. At that time, there was noticeable solar glare in that area of Commonwealth Avenue. This part of Commonwealth Avenue also has a slight curve that you come around and ends at the point where the crash occurred. After, I went back to my cruiser and checked the status of the vehicles and operators through the RMV on my laptop. It showed that Maria Kuznetsova had a expired non renewable driver's license which expired on 07/15/21. Kuznetsova's Honda Civic came back with a status of being unregistered due to revoked insurance. It was revoked on 08/29/21 through the MA RMV. I issued MA Uniform Citation # 185998AB in hand to her for Ch. 90/10/A Unlicensed Operation of a Motor Vehicle, Ch. 90/9/B Unregistered Motor Vehicle, and Ch. 90/34J Uninsured Motor Vehicle and explained the process to her. Motor Vehicle Inventory Tow Form submitted.

Operator of vehicle one ( 2019 Honda CRV MA reg 2SAH91 ) Elizabeth Sylvia stated that on 11/16/2021 at 16:56 hours she was driving south bound on Needham St near Charlemont St. Needham St is a public way in the City of Newton. Sylvia stated that as her vehicle slowed to turn left onto Charlemont St it's front driver's side struck a pedestrian in a crosswalk. Sylvia stated that she did not see the pedestrian prior to the crash due to the area not being lit. It should be noted that the over head street light was inoperable, and the cross walk was not illuminated or reflective. The crosswalk sign that was posted before the crosswalk was not reflective. Sylvia stated that she was unaware that a crosswalk was at the crash location. Sylvia was not injured in the crash , and her vehicle had no damage to it. The pedestrian Nivicha Morpeau stated that she was in the crosswalk in front of 241 Needham St when she was stuck by the front driver's side of vehicle one. It should be noted that Morpeau was wearing dark clothing at the time of the crash. Morpeau complained of back pain, but refused medical treatment and transportation to the hospital. Morpeau singed a patient refusal form provide to her by Medics. Based upon statements made to me by Sylvia and Morpeau I issued Sylvia MA Uniform Citation T 2016746 and cited her for a violation of Newton City ordnance 19/116 failure to stop for a pedestrian in a crosswalk. I took photos of vehicle one and the disk was forward to the NPD's IT Bureau for downloading.

At approximately 2120HRs on Tuesday, November 16, 2021, I was dispatched to the intersection of Hammond St. and Beacon St., for a reported two car crash. Upon arrival, I observed MV1 to have sustained damage to its front passenger's side fender/head light, hood, grille, bumper and it was leaking fluids. I observed MV2 to have sustained damage to its driver's side fender/door and it was leaking fluids. Upon speaking to the operator of MV1, they stated that they were traveling straight (southbound) on Hammond St. and they had a green traffic signal. While they were in the intersection, MV2 failed to stop for their red traffic signal, as they traveled straight on Beacon St. (eastbound) and as a result, MV1 struck MV2. Upon speaking to the operator of MV2, they stated they were traveling straight on Beacon St. (eastbound). At this time, their windshield fogged and they could not see out of it. They continue to drive straight, while attempting to adjust the HVAC controls to clear up the windshield. They then proceeded into the intersection and were struck by MV1. They stated they did not know the color of their traffic signal, as they were looking down at said controls. Neither party were injured. Newton Fire applied speedy dry to the fluids. Todys removed both vehicles. I cleared without further incident and completed NPD Towed Motor Vehicle Forms for each vehicle.

Operator of vehicle one , Yuan Chen stated that while turning onto the west bound on ramp of RT 9 from the north bound lane of Center St the front driver's side bumper of his 2013 Land Rover SUV struck pedestrian Anastassia Javortchouk on her right side. Chen stated that the roadway was not well lit and he did not see Javortchouk in the marked crosswalk. Chen stated that he was not injured in the crash and his vehicle had no visitable damage. Javortchouk stated that she looked both ways before crossing over the RT 9 west bound on ramp in the marked crosswalk heading north bound. Javortchouk stated that she did not see any vehicles approaching and entered the marked crosswalk. Javortchouk stated that she was struck on her right side by vehicle one. Javortchouk had a visible minor laceration to her right leg. It should be noted that Javortchouk was wearing a black shirt and black shorts at the time of the crash, and I observed the lighting conditions to be low. Per Javortchouk's request she was transported via EMS to Boston Medical Center for treatment. Chen was issued MA Uniform Citation T 2016706 and cited for a violation of MGL 89/11, failure to yield to a pedestrian in a crosswalk. All roadways in this report are public ways in the City of Newton.

Operator of MV1 states she was turning left eastbound from Sumner Street onto Commonwealth Avenue when she hit MV2. Operator of MV1 states she couldn't see over the snow pile on Commonwealth Avenue. MV1 was towed by Todys's Service Inc as it was disabled. Operator of MV1 had no injuries and denied medical attention. Operator of MV2 states he was traveling straight ahead (Westbound) when he saw MV1's car lights and then all he remembers was crashing into the fence of 63 Sumner Street. MV2 sustained major damage to the front center and right side of the vehicle. Todys's Service Inc towed MV2 as all the air bags were deployed and the vehicle was disabled. Operator of MV2 had minor injuries and was transported by medics to Newton Wellesley Hospital to be evaluated. A section of 63 Sumner Street fence was damaged due to Motor Vehicle Accident. Owner (Sam Figler) cell phone: 650-269-4780 was informed of the damage. Witness states he saw MV1 come off of Sumner Street and get hit by MV2 on Commonwealth Avenue. Witness' vehicle did not sustain damage. Witness had no injuries. Several photos were taken of accident and turned into IT.

On March 24th, 2022 at approximately 11:24 hours while assigned to N491 I along with NFD and Fallon ambulance responded to the area of #371 Washington St, Honda Village for a report of a MV crash with injuries. On my arrival I located both involved vehicles on Washington St just west of Honda Village. Vehicle #1 was CT tag, C218244, 2020 White GMC 2500 Van with heavy front passenger side damage, operated by a Zachary Sabo He stated he was going W/B on Washington St when vehicle #2 exited out from Honda Village colliding with his vehicle. He further reported there was a large truck double parked W/B in front of Honda village in the non passing lane obstructing his view of vehicle #2 exiting Honda Village as well the view of vehicle #2 as she was attempting to exit out onto Washington St. Vehicle #2 was a 2019 Blue Honda CRV utility, Ma. reg, 4RF-585 with heavy front drivers side damage, operated by a Erin Ryan. Ms Ryan had sustained a head injury so I wasn't able to get a clear statement. She did remember a large truck obstructing her view as she attempted to exit Honda Village onto Washington St. The truck was already gone prior to police arrival. Nobody on scene or Honda Village employees could identify the truck. Both vehicles towed by Todys. Operator to vehicle #2 was transported to Saint Elizabeths Hospital with head and hip injuries.

SHE STATES WHILE BACKING HER VISIBILITY WAS OBSTRUCTED BY THE GLARE OF THE SUN IN THE REAR VIEW MIRROR. AS A RESULT OF THE GLARE SHE COULD NOT SEE THE BLACK COLORED UTILITY GAS LIGHT POLE AND CRASHED INTO IT. THE GAS LIGHT IS LOCATED AT THE FRONT ENTRANCE TO 22 BURNHAM. VEHICLE #1 HAD LEFT REAR DENTS AND SCRAPES. THE GAS LIGHT HAD DAMAGE TO THE TOP CASING AND BULB WHICH HAD SMASHED AND WAS LOCATED ON THE GROUND NEAR THE POLE. CITY OF NEWTON WAS NOTIFIED FOR THE DEBRIS. NATIONAL GRID WAS NOTIFIED AND RESPONDED TO SHUT OFF THE GAS. I TOOK SEVERAL PICTURES OF THE DAMAGE AND GAVE THEM TO THE I.T. DEPARTMENT. DRIVER REPORTED NO INJURIES. SHE WAS ABLE TO LEAVE THE SCENE IN HER VEHICLE WHICH WAS STILL OPERATIONAL. CLEARED WITHOUT FURTHER INCIDENT.

On location I observed the two involved vehicles parked curbside along the westbound side of Washington st directly across from the NWH entrance. V1; 2014 Honda/CR-V Util. color black. The traffic signals at the time were on flash red &yellow instead of normal red, green and yellow. The morning commute traffic on Washington St. is heavy at that time of the morning. V1 operator stated she was waved on by the vehicle operator in the turning lane on the westbound side. V1 proceeded to turn left/westbound onto Washington St. and collided into the driver's side of V2 travelling straight ahead westbound in the outside//32 lane of Washington St. ( westbound). V1 Operator stated she did not see V2. V2 confirmed V1 operator's account of the collision. V2 operator also confirmed the traffic lights were on flash red/yellow. Neither operator was injured as a result of the collision nor was there airbag deployment in either vehicle. Neither vehicle requried towing service.
