Officer A. Kane (MV#2) was parked S/B on Cherry st. @ Pleasant St. processing a past MVA accident. While MV#2 was parked, the vehicle was sideswiped by MV#1 which was travelling S/B on Cherry St. Road conditions were slippery and it was snowing at the time of the crash. MV#2 suffered damage to Drivers side door and drivers front quarter panel. MV#1 suffered damage to front passenger side quarter panel. No injuries reported.

Narrative:
On 05/20/2018 at 17:30 hours I was notified by Captain Anastasia of a motor vehicle crash involving car, and an electric scooter. Captain Anastaisa stated that the operator of the scooter was a 12 year old male who had sustained unknown injures and had been transported to Boston Children's Hospital via ambulance. Upon my arrival at the crash scene I made the following observations, Putnam and Winthrop streets are public ways in the city of Newton that are two way single lane non divided roadways. The roadway surface was wet from rain earlier in the day. Putnam St runs north to south and Winthrop St runs east to west. There is a stop sign at the end of Winthrop St at Putnam St. I observed MA reg 54MN41 a 2005 Buick LeSabre stopped in the south bound lane of Putnam St approximately 1/2 of the way into the intersection with Winthrop St. I observed approximately three feet of skid marks from the Buick's rear tires. I also observed at the crash scene a Segway Ninebot electric scooter that was on the ground near the Buick(cid:31)s rear passenger side door with its handlebars detached. I spoke with the operator of the Buick, Robert DeSouza who stated that on 05/20/2019 at approximately 16:50 hours he was driving south bound on Putnam St at approximately 25 miles per hour. DeSouza stated that he saw a man, later identified as Jose Estabil standing on the east sidewalk of Putnam St facing Winthrop St who was yelling and waving his arms at something on Winthrop St. DeSouza stated he looked to his right towards Winthrop St and his view was obstructed by a row of hedges. DeSouza stated that as he was looking to his right he began to apply his vehicle's brakes when he saw someone coming at me ". DeSouza stated that he heard a crashing noise from the rear passenger's side of his vehicle. DeSouza stated that he stopped his vehicle after the crash and saw a boy lying on the ground at the rear passenger side of his vehicle. DeSouza stated that he was not injured in the crash. DeSouza's vehicle had minor damage to its rear passenger side door, and did not require a tow. Witness Charlotte Hart stated that at approximately 16:30 hours on 05/19/2019 she was diving south bound on Winthrop St. Hart stated that she had to stop short on Putnam St to avoid a crash " with a boy on a scooter that was racing around on Winthrop St. " Hart sated that she told the boy on the scooter that he should not be ridding the scooter in the street. Witness Jose Estabil stated that he was walking on the east side walk of Putnam St across from Winthrop St. Estabil stated that he saw a boy an electric scooter on Winthrop St approaching Putnam St. Estabil stated that he saw DeSouza's vehicle driving south bound on Putnam St approaching Winthrop St. Estabil stated that the boy on the scooter made no effort to stop, slow or avoid the crash with DeSouza's vehicle. Witness Laurel Fransworth stated that she was driving north on Putnam St approaching Winthrop St when she saw the crash occur. Farnsworth stated a boy on an electric scooter on Winthrop St crash into the rear passenger side door of DeSouza's vehicle. Fransworth stated that she believed that the row of hedges at the end of Winthrop St obstructed DeSouz's vehicle. Rowan Delaney ( 02/27/2007 ) stated that on 05/20/2019 at approximately 16:00 hours he and his fellow F.A. Day Middle School student Thiaugo Rubino ( 03/04/2007 ) were having a race around the area of Winthrop St, Putnam St and Shawn St. Delaney stated that he was on foot running on sidewalks while Rubino was on an electric Ninebot Segway scooter. Delaney stated that Rubion was not wearing a helmet or any other safety equipment. I spoke with Delaney(cid:31)s father Royston Delaney, who stated that the scooter was his and that he was unaware that Rubino was using his scooter, and that he has never seen him ridding it before. Royston Delaney stated that the scooter is battery powered and has a maximum speed of 19 MPH. Royston Delaney stated that his son told him that only Rubino was ridding the scooter. Rubino was transported via ambulance to Boston Children(cid:31)s Hospital. On 05/20/2019 Rubino underwent surgery for a brain bleed. I spoke with Rubion(cid:31)s mother Sonia Rubino who stated that as of 05/21/2019 her son was under observation and in addition to the brain surgery her son had a broken arm, broken leg and a broken back. Based upon witnessed statements and my observations of the crash area I believe that Rubion was at fault for the cause of this crash. Myself and Officer Helms took photos of the crash area and the disks were turned over to the NPD(cid:31)s IT bureau for downloading.

MV#1 was traveling north on Centre St. MV#2 was attempting to merge north onto Centre St from east on Hyde St. MV#2 pulled too far into the oncoming lane of traffic causing MV#1 to crash into her. Operator of MV#1 stated he was traveling north on Centre St, and did not see MV#2 until it was to late. Operator of MV#1 stated that he was not able to stop in time and crashed into MV#2. Operator of MV#2 stated that as she was attempting to merge north on Centre St, she was pulling out slowly in an attempt to view the oncoming traffic. Operator of MV#2 stated that she might have pulled to far out, which is why MV#1 struck her. MV#1 was driven off scene, MV#2 was towed away by AAA No injuries occurred.

MV#1 was traveling south on Centre St in the left hand lane. MV#2 was attempting to turn left onto the north side of Centre St from Pleasant St. Operator of MV#1 stated that as she was traveling south, a car pulled out from Pleasant St. Operator of MV#1 stated that there was a large truck in the right lane blocking her view, and she did not see MV#2 pull onto Centre St. MV#1 crashed into MV#2. Operator of MV#2 stated that a truck had stopped in the right lane of Centre St, allowing her to take a left off of Pleasant St. Operator of MV#2 stated that as she was attempting to turn left onto Centre St a MV#1 was in the left lane and crashed into her. Both vehicles were driven off scene. One passenger of MV#2 stubbed her toe during the crash, and was cleared with a patient refusal.

On 07/25/19 at 10:28 hours, I responded to Centre Street and Boylston St off ramp (WB) for a two car motor vehicle accident with no injuries reported. Operator of MV1 states he was traveling straight ahead on Centre Street (NB) when he observed MV2 inching out of the Boylston Street off ramp. He was not able to stop in time and the two vehicles struck one another. I observed heavy damage to the passenger front bumper and tire area and the vehicle was towed by Tody's. Operator of MV2 states he was exiting the Boylston Street off ramp (WB) and was going to attempt a left turn on to Centre Street after stopping for the stop sign. He then slowly pulled out into the intersection due to National Grid trucks blocking his view and that is when he came in contact with MV1 on his driver's side front bumper. I observed moderate damage to the entire front bumper but the vehicle was still driveable. I did observe National Grid trucks parked at the intersection setting up to do construction work. One truck was parked on the left side of the off ramp and the other was parked on the side where the crash occurred. This did obstruct the operator of MV2's view when pulling out.

At approximately 1640 HRs on Wednesday, July, 31, 2019, I responded to the intersection of Centre St. at Hollis St. for a two vehicle crash. The operator of motor vehicle 1 stated she was traveling eastbound on Hollis St. while motor vehicle 2 was attempting to enter Centre St. southbound after stopping at the stop sign controlling the intersection. At this point, motor vehicle 2 reversed and struck motor vehicle 1 at the stop sign. The operator of motor vehicle 2 stated that he was entering the intersection in an attempt to travel southbound on Centre St. when traffic already southbound on Centre St. was flowing. In an attempt not to block northbound traffic, he reversed and accidentally struck motor vehicle 1. motor vehicle 1 sustained damage to its left fender and driver's side door areas. motor vehicle 2 sustained damage to its right rear bumper area and had preexisting scrapes to its right rear quarter and sliding door.. Neither party were injured and both vehicles left under their own power. I cleared without incident.

On Friday, August 16th 2019, at approximately 6:40pm, I, Officer Brooks, responded to Dunstan street at Washington street for a MVA pedestrian. Upon my arrival I spoke with the operator of MV1(294NZ5), who stated she was traveling southbound on Dunstan street and was attempting to pull out onto Washington street, when she made contact with a bicyclist traveling westbound on Washington street. The operator stated she never saw the bicyclist, and that she was traveling less than a mile per hour when she made contact. It should be noted that the sun was setting and the operators vision would have been impaired by the sun at the location of the accident. MV1 had a minor scuff mark on the front bumper. I spoke with the bicyclist who stated he was traveling westbound on Washington street and was crossing over Dunstan street when he was struck by MV1. He stated he was knocked off the bike, scraped his arm, and had minor back pain. The mirror on the right side of his bike was broken. The bicyclist was wearing greenish gray clothing, and was not wearing protective equipment such as a helmet. The bicyclist signed a refusal from Cataldo and was given a copy of the report number. Pictures were taken of the scene and all parties were sent on their way.

VEHICLE #2 DRIVER STATES SHE WAS STOPPED AT THE STOP SIGN TRAVELING N-BOUND ON DUNSTAN. SHE LOOKED TO HER LEFT AND DIDN'T SEE ANY VEHICLES COMING, AS SHE ENTERED THE TRAFFIC LANE SHE WAS UNABLE TO AVOID HITTING VEHICLE #1. DRIVER STATES HER VISIBILITY WAS OBSTRUCTED BY A BLACK PICKUP PARKED ON THE SIDE OF THE ROAD ( MA REG: 7ZV435, 2011 FORD F150 PICKUP ). THE VEHICLE WAS PARKED LEGALLY AND HAD AN ACTIVE REGISTRATION. I WAS STOPPED BY A RESIDENT WHO STATED THERE ARE A LOT OF ACCIDENTS IN THIS AREA BECAUSE OF THE VISIBILITY OBSTRUCTION CAUSED BY VEHICLES PARKING IN THE AREA. VEHICLE #1 HAD RIGHT FRONT SIDE DAMAGE. VEHICLE #2 HAD SEVERE FRONT END DAMAGE, FRONT AIRBAG DEPLOYMENT AND WAS TOWED BY TODYS. BOTH PARTIES WERE EXAMINED BY MEDICS AND SIGNED PATIENT REFUSALS. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES.

Valentine Baron stated on she was bicycling in the marked crosswalk at Center Ave at Center St from the north side walk to the south side walk. Baron stated that she activated the crosswalk light and had entered the crosswalk when the light was activated. Baron stated that while she was in the marked crosswalk vehicle one began to make a right turn on red . Baron stated that vehicle one struck her on her left side , knocking her off her bike . Baron stated that she had a helmutt but was not wearing it the time of the crash. Baron stated that she did not lose consciousness as result of the crash. Baron was transported via EMS to Mt Auburn Hospital for treatment for possible internal injuries. Operator of vehicle one Sara Ross stated that she was stopped at a red light on Center Ave at Center St. Ross stated that she was waiting to turn right on a red light and look to her left and then her right and drove forward. Ross stated that she did not see Baron in the marked crosswalk. Charles Hannum was a front seat passenger in vehicle one and stated that he did not see Baron in front of the vehicle until the crash occurred. I took photos of vehicle one and forwarded to the NPD IT bureau. I issued Ross M Citation T21065501 and cited her for a violation of CO 19/74 , failure to use care while turning.

Narrative:
The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave making a left turn onto Morton St when he was struck by MV#2. The operator of MV#1 stated he was unsure of where MV#2 came from. There was a strong solar glare facing westbound on Commonwealth Ave during this accident. MV#1 sustained heavy driver's side front end damages. Both front airbags were deployed. The operator of MV#1 stated he was not injured. The front passenger of MV#1 was not injured and had already left the scene to attend school. The driver's side rear passenger sustained a bleeding nose from this accident. She was unsure what caused her injuries because it happened too quickly. She was evluated by Newton Paramedics and signed a patient refusal of treatment. The initial point of impact for both vehicles was in the middle intersection of Commonwealth Ave at Morton St. There were broken vehicle parts and tire skid marks starting from the point of impact and onto the berm on Commonwealth Ave on the carriage lane side. MV#2's final resting spot was directly accross from 808 Commonwealth Ave. MV#2 sustained heavy front end damages to its driver's side wheel (Broken axle) and flat front right side tire. MV#2's driver's side airbag was deployed. The operator of MV#2 was transported by Newton Paramedics to the Boston Medical Center. I spoke to the operator of MV#2 via telephone and she stated she was travelling westbound on Commonwealth Ave when she was struck by MV#1 and caused her swerve onto the carriage lane berm. The operator of MV#1 is 6.5 months into her pregnancy and sustained neck, back, bruises and left hand injuries. She was transported to Boston Medical Center and was later discharged the same day. She was provided with this accident report number and was advised to contact her insurance provider. I spoke to the witness who was at the intersection of Cedar St and Commonwealth Ave. The witness stated MV#1 was travelling eastbound on Commonwealth Ave and had stopped, attempting to make a left turn onto Morton St. The witness stated she then observed MV#2 travelling westbound on Commonwealth Ave and struck MV#1. The witness stated she believed MV#2 was travelling at a high rate of speed because it continued to travel onto the berm after striking MV#1. I took digital photographs of the damaged berm on Commonwealth Ave. Based on the statements made to me, I issued in hand to the operator of MV#1 citation #T1268357 for violation of c89 s8 failure to yield when turning left. Both vehicles were towed by "Tody's Towing".

Narrative:
On Wednesday, October 16, 2019, I, Patrolman Hatfield, was assigned to marked cruiser N 495 to Division 5 for the 1530 to 2330 hour shift. At approximately 1742 hours, FTO Guarino and I were dispatched to Commonwealth Ave at Cedar St for a motor vehicle accident with minor injuries. FTO Guarino and I arrived on scene and noticed two MVs with damage. I spoke with the RO/operator of MV2, Janet Haines (Janet). Janet stated she was not suffering from injuries. Janet said she was operating southbound on Cedar St crossing Commonwealth Ave when she had a collision with MV2. Janet stated prior to crossing Commonwealth Ave, an unknown MV stopped at the intersection to allow her to cross. Janet then stated she proceeded accross when she crashed into MV1, who drove around the stopped MV. Janet said her brakes did not work and she continued to drive forward and ended on top of the curb of Commonwealth Ave facing eastbound. MV2 suffered damage to the whole front bumper and wheel wells. FTO Guarino spoke with the operator of MV1, Madeleine Richard (Madeleine). Be advised both front and side driver airbags were deployed, and the front driver's side door was unable to be opened. I then spoke with Madeleine. Madeleine stated she was operating eastbound on Commonwealth Ave before being involved in the collision. Madeleine stated she drove around the MV infront of her that was attempting to take a left from Commonwealth Ave onto Cedar St, and she did not see MV2 prior to the accident. MV1 was struck on the driver's side by MV2, and ended facing westbound in the middle of Commonwealth Ave. EMTs and fire arrived on scene. Janet stated she did not need to be transported. Madeleine was transported to Newton Wellesley Hospital for an evaluation. Both MVs were towed by Todys.

Narrative:
On Wednesday October 23, 2019, I, Patrolman Hatfield, was assigned to marked cruiser N495 in full uniform to Division 5 for the 1530 to 2330 hour shift. At approximately 2040 hours, myself and Officer Selig were traveling northbound on Centre St (a public way) in Newton Center. We were behind MA reg 9KT873 (2004 Toyota Corolla grey). We observed the motor vehicle drive around a pedestrian in the crosswalk who was attempting to cross at Centre St and Pelham St. I activated my emergency blue lights and attempted to conduct a motor vehicle stop of the Toyota Corolla. The operator continued to drive northbound on Centre St before pulling over just before the crosswalk at Centre St and Pleasant St. The operator then continued driving forward and struck a pedestrian crossing in the crosswalk Officer Selig and I saw the pedestrian being struck before landing on top of the Toyota Corolla hood, and falling forward onto the street. Officer Selig and I spoke with the two parties involved. I spoke with the victim, who was identified as Vivian Rubinstein (Vivian). Vivian crawled towards the sidewalk of Centre St while clutching her lower back. Initially Vivian denied being injured and refused medical aid. The Toyota Corolla operator was identified as the register owner Elise Ruben (Elise). Elise stated to Officer Selig that her front windshield was foggy and when she noticed the blue lights, she attempted to pull over. Elise stated she did not see the pedestrian in the crosswalk. Elise appeared very upset and refused medical treatment. Officer Selig was able to speak to two witnesses of the accident. The witnesses were identified as Stephan Aparicio (Stephan) and Mikaela Rice (Mikaela). Stephan said he was behind the victim in the same crosswalk. Stephan stated he saw the Toyota stop in front of the crosswalk and then drove forward hitting the female. Mikaela stated she was parked in her motor vehicle on Centre St facing northbound (facing the crosswalk). Mikaela stated she saw the female walking halfway across the crosswalk and the Toyota continuing drive forward before hitting the female. Officer Siegel arrived on scene and took four photographs which was submitted with this report. Vivian was transported by Newton Medics to Faulkner Hospital for an evaluation. I issued written warning citation number T2079786 to Elise for failure to yield to a pedestrian in crosswalk (Centre St at Pleasant St). I also issued Elise citation number T2079787 for failure to yield to a pedestrian in crosswalk (Centre St at Pelham St). The vehicle was driven from the scene.

The operator of MV#1 stated he was traveling westbound on Franklin Street and came to a stop at the stop sign. He stated he proceeded to cross the intersection at Waverley ave when he struck MV#2. The operator of MV#2 was traveling southbound on Waverly ave when she was struck in the drivers side by MV#1. MV#2 had airbag deployment and the operator was transported by the medics to NWH. There was a construction detour just being set up at the time of the accident. It is possible that the view was obstructed for the operator of MV#1.

Narrative:
On 02/26/2020 I responded to the area of Eastbourne Rd and Ireland Rd for a motor vehicle crash involving a pedestrian. Eastbourne RD is a public way which runs from Ward St north bound intersecting with Ireland Rd on the far north end. Ireland RD is a public way which runs west to east. Both the east end and the west end of Ireland Rd are dead ends. The crash occured on Ireland Rd's east end near the intersection of Eastbourne Rd. The crash involved Vehicle 1 described as a 2016 Acura SUV color grey bearing MA passenger registration 3RB127. Vehicle 1 had a valid inspection sticker which expires on 08/30/2020. Vehicle 1 was operated by its owner identified as Barbara Ball. Ball has an active MA Operators License (S18727836). The involved pedestrian was identified as Bruce Terrio. Terrio is an employee of Feeney Brothers Utility Services. Feeney Brothers Utility Services is a contractor of National Grid Gas Company. Upon arrival I observed the following: The crash occurred in the daylight hours around 10:30am on Ireland Rd near the intersection of Eastbourne Rd. At the time of the crash the weather conditions were clear, 48 degrees Fahrenheit and overcast skies (weather.gov). The area where the crash occurred was an active construction site for Feeney Brothers Utility Services. The area of the crash is a dead end. The area is accessed from Ward St from either Eastbourne Rd or Coolidge Rd. The entrances of both Eastbourne Rd and Coolidge Rd had construction signs indicating utility work in the area. At the time of the crash, all roadways were open to vehicle traffic. On the eastbound side of Eastbourne Rd just prior to the intersection of Ireland Rd, I observed a large air compressor which was parked in the roadway facing south bound. No traffic cones were located near the air compressor. To the right of the air compressor were thick green bushes. A wooden fence was located to the right side of the green bushes. On the east side of Ireland Rd I observed a large excavated hole on the right side of the roadway. The hole was near the intersection of Eastbourne Rd and was located approximately three feet from the curb. I observed four traffic cones, two on the front side and two on the left side of the hole. Located on the front edge of the hole was a tire print. Vehicle 1 was located on the west end of Ireland Rd. The front passenger tire of Vehicle 1 had fresh dirt located on the sidewalls and the tire tread. Vehicle 1 appeared to have no visible damage. The pedestrian (Terrio) was being treated by EMS and Fire for injuries. Terrio was wearing brown pants and a green reflective safety vest. I spoke with the operator of Vehicle 1 Barbara Ball. Ball stated that she was operating Vehicle 1 northbound on Eastbourne Rd near the intersection of Ireland Rd. Ball stated that she made a right turn onto Ireland Rd and drove on to the edge of the excavated hole. Ball stated that her vehicle then made impact with Terrio who was in the hole. Ball stated that she immediately stopped, backed out of the hole and called 911. Ball stated that prior to making a right turn onto Ireland Rd, there were no traffic cones in the roadway or next to the hole. Ball stated that as she made the right turn onto Ireland Rd she did not observe Terrio or the hole until it was too late. Ball states that she resides at 80 Eastbourne Rd which is the residence located to the immediate right of the crash location. Ball states that she had been recently parking on Ireland Rd due to ongoing construction at her residence. Ball states that she was traveling at a slow speed prior to impact. At the time of the crash there were three other Feeney Brothers employees working at the site. These workers were identified as Steven Gamble, Mike Cabrinma and Gerard McComaskey. At the time of the crash all three workers were located further up Eastbourne Rd near the intersection of Coolidge Rd and did not observe the crash. At the time of the crash, Newton Police Sergeant Glenn Chisholm was working a paid detail for Feeney Brothers. Sergeant Chisholm stated that he did not observe the crash.Terrio was transported by Cataldo EMS for minor injuries. Several photos were taken of the scene. Photos were taken of the hole with traffic cones and without traffic cones. All photo's were submitted to the IT Bureau. After clearing the scene, I responded to St Elizabeth's Hospital in Brighton, MA to interview Terrio. Terrio stated that at the time of the crash he was standing in the hole on Ireland Rd performing gas utility work. Terrio stated that the front passenger side bumper of Vehicle 1 struck him in the right shoulder. Terrio stated that the impact caused him to lose his balance and fall down in the hole. Terrio states that after falling down he was able to climb out of the hole without assistance. Terrio stated that at the time of impact he was wearing a white hard hat which he thinks struck the pavement on the edge of the hole. Terrio states that at the time of the crash he was working alone in the hole. Terrio states that at the time of the crash traffic cones were located near the hole. Terrio stated that as a result of the impact he sustained minor injuries to his right shoulder and back. I then observed Terrio's hard hat which appeared to have a fresh scrape mark on the top. The hard hat also had broken interior straps. Terrio stated that the straps broke when he was struck by Vehicle 1. Photos were taken of the hard hat and were submitted to the IT Bureau. After reviewing all facts of this incident, Vehicle 1 appeared to have been traveling at a slow speed prior to the crash. This was evident due to the lack of tire skid marks prior to the tire print on the edge of the hole. It also appears that Ball's view of the hole and Terrio may have been obstructed by the parked air compressor on Eastbourne Rd. The thick green bushes and a wooden fence to the right of the air compressor also added to the obstructed view. The hole was also located in close proximity to the air compressor which reduced Vehicle 1's reaction time. Jeff Oliver of OSHA was also notified of this incident. Operator of MV1 states he was traveling westbound on Commonwealth Ave when he attempted to take a left turn onto Fuller Street and struck a bicyclist who was traveling eastbound on Commonwealth Ave. Operator of MV1 states he didn't see the bicyclist due to the solar glare from the sun obstructing his view of the roadway. It should be noted the sun was setting in the west at this time. MV1 sustained minor damage to the windshield and front right fender. No injuries reported by the operator of MV1. The bicyclist, Martin Pleynet, states he was riding his bicycle eastbound on Commonwealth Ave traveling towards the Boston College area when he was struck by MV1. Mr. Pleynet states he doesn't have recollection of the accident since he lost consciousness for a brief time after being struck. Mr. Pleynet was transported to Brigham and Women's Hospital where he sustained a fractured left shoulder, abrasion on his right shin and abrasion on his forehead. The bicycle sustained damage to the front wheel and was placed into property and evidence for safekeeping. Pictures of the scene were captured and the SIM card was submitted to the IT bureau.

On Thursday 6/24/21 I responded to a two car motor vehicle accident at the intersection of Lowell Ave and Washington st. Upon my arrival I spoke with the operator of MV 1 (RI REG CONCPT) who stated he was headed westbound on Washington and trying to turn southbound on Lowell Ave. He said his view was obstructed by an eversource detail that was working just before the intersection on Washington st. When he was making the turn he was struck by MV 2 (MA REG 2LBM55) which had been traveling eastbound on Washington st. MV 1 had damage on the right side back passenger door. I spoke to the operator of MV 2 who stated she was traveling eastbound on Washington St when MV 1 cut in front of her even though she had the right of way. MV 2 had damage to the front end and airbag deployment. The driver of MV 1 had no injuries and MV 1 was driven away from the scene. The operator of MV 2 was transported to Newton Wellesley Hospital and her vehicle was towed by Tody's.

Operator stated he was driving west on Charlesbank Rd and was attempting to make a left turn onto St James St when he struck the concrete island barrier that divides east and west traffic to Charelesbank Rd. Operator further stated due to his windshield fogging up from the weather/heavy rain he didn't see the island barrier. This vehicle I found out after the operator left with a LYFT driver was a short time rental from KYLAS Auto body, #819 MAIN St Brockton Ma. I contacted that business and spoke with the receptionist, named Ariann. I informed her about the accident and that I didn't identify the operator for my report. She was going to attempt to contact the person who rented the vehicle and get back to me. The person that rented the vehicle was a woman out of Brockton. KYLAS Auto was going to contact her to identify the person who she allowed to drive the vehicle this morning. The woman identified by KYLA'S Auto body as the person who rented the involved vehicle was a Maria Amado, 234 Green St, Brockton Ma. 02301. Kyla's Auto body will be going after her insurance carrier for the damage to their vehicle.

Operator of vehicle one Sara Kolaczyk stated that while turning right from Albemarle Rd onto Crafts St her vehicle's front drive's side quarter panel struck pedestrian Mariana Pishtikova in the marked crosswalk. Kolaczyk stated that she was looking to her left at on coming traffic on Crafts St and had began to turn right when crash occurred. Kolaczyk stated that she did not see Pishtikova before the crash. Pishitkova stated that she entered the marked crosswalk at Crafts St and Albemarle Rd after she saw that Kolaczyk vehicle had stopped. Pishtikova stated that she walked around the front of Kolaczyk's vehicle and it then turned into her at a very low speed. Pishtikova stated that the front drivers side quarter panel of Kolaczyk's vehicle struck her right side. Pishtikova stated that her right ankle was sore but refused medical treatment. Pishtikova signed a patient refusal form provided to her by Newton Medic Unit Three. I took photos of Kolaczyk's vehicle and the disk was downloaded by the NPD's IT bureau. Kolaczyk's vehicle had no damage and did not require a tow. Kolaczyk stated that she was not injured in the crash. I issued Kolaczyk MA Uniform Citation T2016945 and cited her for a violation of MGL 89/11 , failure to yield to a pedestrian in a crosswalk. Crafts St and Albemarle Rd are public ways in the City of Newton.

On 09/10/2021 at 16oo hrs I responded to the area of Washington St@Belmore PK for an MVA. I arrived on location and observed V1 NY Reg SX77; 2019 Volvo/LL color blue. V1 Operator NY OLN 968528076. V2 MA Reg 389ME3; 2015 Niss/Altima color black. Both operators were out of the vehicles upon my arrival. V2 was having chest, back and neck pain. V2 operator was evaluated by EMS on scene. V2 Operator was transported by ambulance to Newton Wellesley Hospital for further evaluation. V1 operator was uninjured and sign an evaluation waiver with EMS on scene. On scene I spoke with V1 operator who stated he was coming off the (5N off ramp attempting to turn left onto Washington St. (westbound). Both directions of Washington are multi lane. V1 operator described traffic as heavy. V1 operator stated he saw a gap in traffic and proceeded to cross over the eastbound lanes to turn left onto the westbound side. V1 operator stated he never saw V2 traveling in the #1 lane of Washington St (eastbound). V1 had an obstructed view by oncoming traffic in the #2 lane of Washington St. (eastbound) and collided into V2. The crash scene was cleared and both vehicles were towed by Tody's Towing Inc. I did not observe airbag deployment in either vehicle. Once clear of the crash scene I proceeded to NWH to speak with the V2 operator. V2 operator stated she was traveling straight ahead on Washington St (eastbound). V2 operator basically explained never knew what hit her. She never saw V1 and only felt the impact of the collision. V2 operator was waiting to be treated for chest, back and neck pain. V2 was placed in a cervical collar and back board by EMS prior to transport to NWH.

On 10/14/2021 while assigned to marked cruiser N493 at approximately 18:34 hours I, Officer Vazhenin, responded to 307 Auburn Street for a report of 2 car MVA. Upon arrival I spoke to Ms. Jennifer Ann Pope (Driver 1 of MV 1), who stated that when she was taking a left turn to Auburn Street, she followed a car (MV 3) in front of her, because she believed that MV 2 saw her and was letting her to take a left turn. Ms. Pope (Driver 1) said that when she was making that turn, MV 2 started making a turn in the same direction. She stated that she stopped, but MV2 kept driving and crashed into the right front corner of her car (MV1). I then spoke to Ms. Katherine Mary Iodice (Driver 2 of MV2), who stated that she yielded the right of way to MV3 and then started making a turn right after that car. Ms. Iodice stated that she did not see MV1, which was following closely to MV3, and while she was making a turn, she observed MV1, but it was too late to stop and her car (MV2) crashed into MV1. Driver 1 of MV1 was at fault because she failed to yield the right of way to Driver 2 of MV2, which had an obstructed vision and did not see MV1.

At approximately 2120HRs on Tuesday, November 16, 2021, I was dispatched to the intersection of Hammond St. and Beacon St., for a reported two car crash. Upon arrival, I observed MV1 to have sustained damage to its front passenger's side fender/head light, hood, grille, bumper and it was leaking fluids. I observed MV2 to have sustained damage to its driver's side fender/door and it was leaking fluids. Upon speaking to the operator of MV1, they stated that they were traveling straight (southbound) on Hammond St. and they had a green traffic signal. While they were in the intersection, MV2 failed to stop for their red traffic signal, as they traveled straight on Beacon St. (eastbound) and as a result, MV1 struck MV2. Upon speaking to the operator of MV2, they stated they were traveling straight on Beacon St. (eastbound). At this time, their windshield fogged and they could not see out of it. They continue to drive straight, while attempting to adjust the HVAC controls to clear up the windshield. They then proceeded into the intersection and were struck by MV1. They stated they did not know the color of their traffic signal, as they were looking down at said controls. Neither party were injured. Newton Fire applied speedy dry to the fluids. Todys removed both vehicles. I cleared without further incident and completed NPD Towed Motor Vehicle Forms for each vehicle.

Operator of vehicle one , Yuan Chen stated that while turning onto the west bound on ramp of RT 9 from the north bound lane of Center St the front driver's side bumper of his 2013 Land Rover SUV struck pedestrian Anastassia Javortchouk on her right side. Chen stated that the roadway was not well lit and he did not see Javortchouk in the marked crosswalk. Chen stated that he was not injured in the crash and his vehicle had no visitable damage. Javortchouk stated that she looked both ways before crossing over the RT 9 west bound on ramp in the marked crosswalk heading north bound. Javortchouk stated that she did not see any vehicles approaching and entered the marked crosswalk. Javortchouk stated that she was struck on her right side by vehicle one. Javortchouk had a visible minor laceration to her right leg. It should be noted that Javortchouk was wearing a black shirt and black shorts at the time of the crash, and I observed the lighting conditions to be low. Per Javortchouk's request she was transported via EMS to Boston Medical Center for treatment. Chen was issued MA Uniform Citation T 2016706 and cited for a violation of MGL 89/11, failure to yield to a pedestrian in a crosswalk. All roadways in this report are public ways in the City of Newton.

Operator of MV2 states he was traveling straight ahead (Westbound) when he saw MV1's car lights and then all he remembers was crashing into the fence of 63 Sumner Street. MV2 sustained major damage to the front center and right side of the vehicle. Todys's Service Inc towed MV2 as all the air bags were deployed and the vehicle was disabled. Operator of MV2 had minor injuries and was transported by medics to Newton Wellesley Hospital to be evaluated. A section of 63 Sumner Street fence was damaged due to Motor Vehicle Accident. Owner (Sam Figler) cell phone: 650-269-4780 was informed of the damage. Witness states he saw MV1 come off of Sumner Street and get hit by MV2 on Commonwealth Avenue. Witness' vehicle did not sustain damage. Witness had no injuries. Several photos were taken of accident and turned into IT.

MV2 drivers window was broken. Tody's towing responded and towed both vehicles involved. A towed motor vehicle form was filled out for both and placed on file.

Operator of MV1 states she was traveling eastbound on Beacon Street approaching the intersection of Centre Street when she came to a stop at the red traffic light. Operator of MV1 states when the traffic light turned green she accelerated and struck a male party on a recumbent trike with the front bumper of MV1. Operator of MV1 states her visibility was obstructed from the hood of her vehicle because the recumbent trike is very low to the ground. It should be noted the trike had a flag attached to it, which the operator of MV1 stated she didn't see until after the accident happened. MV1 sustained minor damage to front bumpers license plate. No injuries. The bicyclist, Christopher Tsu, states he was traveling eastbound on Beacon Street on his recumbent trike when he came to a stop at the red traffic light at Centre Street. Mr. Tsu states when the traffic light turned green, MV1 struck him in the rear of his trike causing him to fall to the ground. Fallon Medic 5 evaluated Mr. Tsu and he signed a patient refusal. No damage done to the trike and no injuries.

ON 5-22-22 AT APPROX. 1848HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT 25 CRAFTS ST. I SPOKE TO THE DRIVER OF VEHICLE #1. DRIVER STATES SHE WAS HEADED N-BOUND ON CRAFTS WHEN VEHICLE #2 ENTERED THE TRAFFIC LANE AND HIT HER LEFT SIDE CAUSING DAMAGE. VEHICLE #2 STATES SHE WAS EXITING WHOLE FOODS FROM THE CRAFTS ST. EXIT GOING LEFT ON CRAFTS. SHE STATED WHILE EXITING SHE HIT VEHICLE #1 WHO HAD CUT IN FRONT OF HER. BOTH PARTIES HAD THEIR VISIBILITY OBSTRUCTED BECAUSE NEITHER VEHICLE SAW EACH OTHER UNTIL THEY CRASHED. . VEHICLE #1 HAD LEFT SIDE DAMAGE AND WAS STILL OPERATIONAL. VEHICLE #2 HAD FRONT END DAMAGE AND WAS TOWED BY AAA. BOTH PARTIES REPORTED NO INJURIES AND SIGNED PATIENT REFUSALS. BOTH PARTIES ADVISED TO CONTACT THEIR INSURANCE AGENCIES. CLEARED WITHOUT FURTHER INCIDENT.

MV 1 was traveling westbound on Watertown St and attempting to make a right turn on Wildwood Ave. The operator of MV 1 stated a vehicle traveling in the opposite directions high beams obstructed his visibility and caused him to make the turn too sharply. MV 1 struck the curb at the intersection of Watertown St and Wildwood Ave with his right front tire. MV 1 sustained significant damage to the right front wheel and tire rendering the vehicle disabled. Tody's responded on scene and towed the vehicle away. The operator of MV 1 was not injured in the accident. The operator, Mr. Manuel Rodrigues, was issued citation T1448398 for MGL ch 90-9 unregistered motor vehicle because the vehicle he was driving did not have an active registration.
