On 1/5/19 at 1210hrs I was dispatched to the Pelham St Municipal Lot (public way inviting access maintained by the City of Newton) behind CVS for a report of an MVA involving several vehicles. On arrival, I observed Mv#1 MA Reg 5TWS10 with heavy front end damage wedged in between two other vehicles parked in marked spaces. #1 operator, in the driver's seat, was being evaluated by Medics. #1 was identified as Mary Jane Hymanwegscheider (10/14/38). I first made contact with a witness identified as Sharon Corliss. Ms Corliss stated she was sitting in her parked vehicle and observed Mv#1 backing out of the end spot next to CVS. She explained once #1 had came to a stop from backing, #1 then pulled forward, drove over a curb island and continued driving straight into the two parked vehicles. Ms Corliss stated it appeared #1 was travelling about 5mph preceeding the crash. After the medics intial evaluation of Ms Hymanwegscheider, I was able to ask her what happened? She stated she backed out of her space, hit the curb going forward and then put her foot on the gas instead of the brake before the collision. She was deliberate when speaking and appeared confused. She reported to medics she had no medical history and was not using any medications including allergy medication as well. She reported she did not hit her head after the collision. She was then transported to NWH for further evaluation after medics discovered her blood/sugar readings were very low. I advised Ms Hymanwegscheider her vehicle had heavy damage and was being towed by Tody's and to expect paperwork at the conclusion of the crash investigation. It appeared #1 struck Mv#2 first on its driver's side rear before striking Mv#3 parked next to it on the passenger side rear. #1 wedged itself between #2 and #3, #3 was then pushed to the left into the passenger side of Mv#4. #2 and #3 sustained heavy rear end damage. #4 sustained light damage. #1 towed by Tody's with heavy front end damage. #2 arranged for her own tow through her insurance company. A note was left on #3, and #4 was able to be driven from the scene. Ms Hymanwegscheider is showing no MV infraction/no accident history. Based on the totality of all information received as a result of this investigation, I will be issuing a citation (#T0340410) to Ms Hymanwegscheider for Ch90/Sec24, Negligent Operation.

On Monday, February 11th, 2019, I responded to a two vehicle MV accident in the parking garage at Lifetime Fitness Center at 300 Boylston St. There were no injuries. Vehicle # 1 (Bearing Mass Reg # 16KK69, Toyota Rav4, owned by Sasha Bordett) was unoccupied and parked in a marked spot facing in. Vehicle # 2 ( Bearing Mass Reg #875LD0, Toyota SUV, owned/operated by Cassandra Spittel) was parked backed in to the spot next to vehicle # 1. Cassandra stated she took a right hand turn out of the parking spot and heard a noise. She stopped her vehicle and inspected the damage and saw scratches to the right front side of her vehicle. She also noticed a dent to the right rear of Bordett's vehicle. Since the other vehicle was unoccupied, Cassandra called Newton Police and stated she had been in an accident and left her info to myself to make an accident report. I contacted the owner of the Toyota Rav4 and informed her of the accident and informed her their would be an accident report on file.

On 5/20/19 at about 1510hrs I was dispatched to 136 Evelyn Rd for an MVA. En route to the location with Ofc Gourdeau, it was determined by first arriving Ofc Gourdeau that a vehicle had crashed through a fence at the end of Troy Lane and came to rest in the backyard of 126 Evelyn Rd. On arrival, I located the vehicle operator, identified as Mr Stuart Phillips who was operating MA Reg 8VVP40, an '03 Jaguar S Type model. Mr Phillips was walking around the backyard after the collision and appeared coherent as to what had transpired. He sustained a laceration to the top of his head and was bleeding heavily. We sat him down in a lawn chair prior to medics arrival to evaluate. When asked what happened, he stated he "hit the gas instead of the brakes". I asked him where did the accident begin? He stated he was turning around at the end of Troy Lane (a dead end cul de sac) and crashed through the fence there. Mr Phillips was alert and speaking clearly. I asked him if he was on any medication? He stated he was taking Plavix only, a blood thinner. I asked about his medical history and he stated he was released from Spalding Rehab today for treatment from a stroke and was receiving occupational therapy. Medics arrived to evaluate and he was transported to NWH for treatment. An investigation of the crash path of the vehicle revealed the following based on tire tracks and damage left behind: The vehicle crashed through the fence at the end of the cul de sac belonging to 41A Helene Rd; Vehicle drove over that back lawn, clipped the back deck attached to the rear of the house; damaging the base of the deck and lattice below; Vehicle travelled over the patio, then crashed through shrubbery down an embankment leading to the backyard of 132 Evelyn Rd; Vehicle drove through backyard of 132 Evelyn, then continued along the side of the house where it drove over damaging three retaining walls and clipped a large tree; After driving over the third retaining wall, vehicle continued through the backyard of 126 Evelyn, causing significant lawn damage; Vehicle continued until it hit the back of the house at 126 Evelyn; there, it drove over the rear patio before it struck a ground level basement window and window well; It came to a rest there where the vehicle caught on fire under the hood. Witnesses helped extinguish the fire with a garden house (see incident report #19020129) until NFD arrived to knock it down. Significant front lawn damage to 126 Evelyn occurred as well as NFD was arriving on scene to what was first reported as a structure fire. Several photos of the area were taken by myself. The memory chip was TOT the I.T. bureau to be attached to this report. Tody's towing towed the vehicle out of the backyard by accessing the rear driveway of 132 Evelyn Rd. Mr Phillips cited for Negligent Operation and an Immediate Threat for License Suspension/Revocation faxed to the RMV. Troy Lane is a public way in the City of Newton.

RP David Alessi who is an employee for the chestnut hill mall stated he was alerted to a light post down outside of the mall in the parking lot. David stated he met with operator of MV#1 that stated he was backing up did not see the light poll behind him. He struck the light poll causing it to uprooted and fall over. MV#1 was not on scene I gathered the information from RP David. MV#1 was operating a garbage truck for company JRM whom is subcontracted out with the mall. I was not able to reach the drive, he will not be issued a citation. I then cleared with no further incident.

It should be noted MV1 was not parked in their space properly (had a foot or 2 to pull forward and their drivers side tire was completely on the yellow line). I CONTACTED WESTWOOD PD AND THEY HAD AN OFFICER RESPOIND TIO THEIR ADDRESS TO NOTIFY THE OWNER OF THE MV TO CALL NEWTON PD REGARDING THE ACCIDENT. JANE TALCOTT CALLED ME AND SHE WAS ADVISED OF THE MINOR MV ACCIDENT SHE WAS IN AND SHE WAS ADVISED TO CONTACT HER INSURANCE COMPANY.

On Friday, October25th, 2019 at approximately 1635 hours I, Officer Newton and Officer Coletti, were dispatched to the parking garage at 300 Boylston St. for a motor vehicle accident. Upon arrival I spoke with the operator of vehicle 1. He stated as he was heading towards the exit, vehicle 2 suddenly stopped in front of him. Vehicle 2 then backed into him and started moving forward again. The operator of vehicle 1 then exited the car to stop vehicle 2. The Operator of vehicle 2 then got out of her vehicle. The operator of motor vehicle 2 stated she was driving and she stopped to back up, as she backed up she thought she hit a curb but then the operator of vehicle 1 was at her window saying she hit him. As a result of the crash vehicle one sustained very minor front end damage and vehicle 2 had no noticable damage.

Operator of Vehicle #1 stated that he was backing out of a parking spot on Floral street and gently tapped Vehicle #2. Operator states that a vehicle was parked closely in front of his vehicle making it difficult for him to pull out of the parking spot. I observed no damage to Vehicle #1. On Monday, November 4th, 2019 at approximately 1557 hours I, Officer Newton and Officer Healy responded to the municipal parking lot at 121 Adams St for a motor vehicle accident. Upon arrival I spoke to the operator of vehicle 1. She stated she was sitting in her parked car when vehicle 2 backed out of his spot and struck the front of her car. I observed fresh damage to the driver's side bumper. The operator of vehicle 2 told the operator of vehicle 1 he would be right back. Vehicle 2 then left the parking lot and never returned. It should be noted that this vehicle was involved in another collision after this one. The operator of vehicle 2 stated he did not remember this accident when we asked him. As a result of my investigation, Mr Usher was issued Citation#T2079940 for a violation of M.G.L.Ch.90/24-Leaving the Scene Property Damage. Based on the totality of the circumstances, an Immediate Threat Request was faxed to the RMV, see Incident number 19047444.

On Thursday, November 14, 2019, while assigned to Traffic unit N525, I responded to the rear parking lot of the Angier Elementary School located at 1697 Beacon Street for a motor vehicle crash involved a City of Newton owner vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. The operator of MV1, Jessica Moore, stated she was backing out of a parking spot in the rear of the Angier Elementary School while operating a 2007 Chrysler Town and Country (MA:395KR9). Moore stated she did not see MV2 passing by as she was backing out and the rear driver side bumper area of her vehicle crashed into the rear driver side of MV2. Moore reported no injuries. I observed moderate damage to the rear driver side bumper area of MV1. The operator of MV2, Patrice Lys, stated he was traveling Westbound in the parking lot of the Angier Elementary school in a 2018 Ford EcoVan (MA MVN: M2546A). Lys stated a vehicle on his driver side backed out of a parking spot and the rear end of MV1 crashed into the rear driver side of MV2. Lys reported no injuries. I observed minor damage to the rear driver side area of MV2. The operator of MV1 was cited with Massachusetts Uniform Citation T2016428 for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care, Backing). Photos were submitted to the IT Bureau.

On Wednesday, November 20, 2019, I met with Officer Lazarakis at 25 Chestnut Street, Newton for a report of damage to Newton Police unmarked cruiser 1325. Officer Lazarakis stated she was retrieving an item out of the front passenger side door of the 2017 Ford Fusion (MA: 7HD877) when she observed a medium size dent near the door. Officer Lazarakis stated she does not know when or where the damage occurred. The vehicle has been moved several times in the last few days. The vehicle is typically parked in the rear parking lot of 25 Chestnut Street. It appears the dent was caused by a vehicle backing into the side of the vehicle. I observed no paint transfer or any other physical evidence left by the unknown vehicle. Pictures were taken to document the damage to the front passenger side door and submitted to the IT Bureau. IT was asked to check the surveillance cameras at 25 Chestnut Street to see if the incident was captured. I checked the area for vehicles for damage consistent to the damage on 1325 with a negative result.

MV#1 was parked next to handicapped stalls by the front doors of 1 Boyston Street when it was struck by MV#2. -OpMV1 stated he left his vehicle next to the handicapped stalls by the front doors of 1 Boylston Street approximately 1630hrs. He stated he returned to the stall, approximately 1730, where he discovered his vehicle had been struck in the rear right panel. OpMv#1 showed me the pictures of the damage to the vehicle on his phone. He stated he wanted to get the video surveillance of the parking lot before reporting the incident. His vehicle was not towed from the scene and no other incidents matching his collision were reported to the police.

Narrative:
Operator of Vehicle #1 was traveling on Saw Mill Brook Parkway before turning right over a curb and onto Kappius Path. Kappius Path is a walking path not intended for vehicle traffic. While traveling on Kappius Path the operator struck a tree with the driver's side front of the vehicle. The vehicle then rolled onto the passenger side. In order to extract both occupants of the vehicle, Newton Firefighters used the Jaws of Life to remove the roof of the vehicle. Firefighters also had to remove several sections of a chain link fence belonging to the property owner of 19 Kappius Path. Newton Medics obtained patient refusals from both occupants. No injuries were reported. The vehicle was towed from the scene by Todys and an inventory sheet was completed by Officer Emmanuel. Five photographs of the scene were submitted to the IT Bureau. The operator of Vehicle #1 was subsequently arrested and charged with OUI, Negligent Operation and Marked Lanes.

Ms. Cooney states that she pulled into a parking space at Marshall's Department Store and when attempting to hit the brake she accidently hit the accelerator causing her vehicle to jump the curb, crash into the fence, and become stuck on top of a bush. The damage to the fence is estimated at approximately $1,000. Ms. Cooney reported no injuries and Tody's responded to the scene and removed the car from the berm. The vehicle did not sustain any apparent damage at this time and was driven from the scene. I provided the manager of Marshall's, Adriana Guara, with a copy of the crash report number and contacted the building's landlord, Dana, and updated him on how to obtain a copy of the crash report.

This was witnessed by two witnesses who ran outside and the car was gone. They were able to give dispatch plate of vehicle # 2 which matches description of a tan SUV with a woman driver. Due to very minor damage (small scrapes), Oper # 2 will not be charged with leaving the scene, she may have not been aware of damage) and RP (owners daughter) agreed that she just wanted it documented. Attempted to locate MV #2 at her home but was unsuccessful. Also tried phone number given and it wasn't answered and I wasn't able to leave a voicemail.

MV#1 was parked in a stall of the parking lot of 199 Boylston Street when it was struck by MV#2. The owner and operator of MV#2 left the area, however, during an investigation of the mater the owner and driver was identified and located. -OpMv#1 stated that approximately 1140 she parked her vehicle in a parking stall in the parking lot of 199 Boylston Street. She stated she returned, approximately, an hour later and discovered the damage to her vehicle. -OpMv#2 stated, via telephone, she was at 199 Boylston Street, was operating MV#2, however,that she did not know she had struck Mv#2. She stated it was a very tight parking space but did park in the stall. - While at the scene I observed a scrape on the door of MV#1 front right passenger door. I next observed that MV#2 very close to MV#1 making it very difficult to move between vehicles. I also observed that MV#2 was parked very far forward than the vehicle on the right. I observed a scrape on the front bumper with traces of white paint into it; it appeared to be paint transference. -Neither vehicle was towed from the scene and since neither driver was in the vehicle at the time of the collision there were no injuries reported.

On 2/3/2020 at approx 1114hrs while assigned to 497 I responded to 2370 Commonwealth Av, The Speedway Gas Station for a report of a two car crash in the parking lot w/o injuries. Upon arrival I met with the owner of Ma Reg 25WR69, Anthony LUONI who stated he was stopped at the gas pump when the operator of Ma Reg 3672310 , Joshua RICHARDSON who was trailering u-haul trailerTX Reg AO37011 backed into him . RICHARDSON admitted he was attempting to back up and align with the gas pump when he accidently struck LUONIS car. RICHARDSON additionally admitted to having an expired license and registration out of NH and no insurance, RICHARDSON also had a valid rental agreement for the UHAUL trailer TX reg AO37011 with insurance . RICHARDSON was able to re register his van on scene and provided me with an emailed copy of confirmation of registration renewal and his passenger Brendan SEIGAL who had a valid NH license was able to drive . RICHARDSON issued a criminal application on Mass Uniform Citation T2015308 for violation of MGL 90/10 Unlicensed Operation and 90/9 Operating Unregistered MV .

A partial Mass Reg of 21D81 was provided. A message was left for the owner of (MA REG: 2LD812) as that is the closest plate and vehicle is registered in close proximity to where the incident occurred. Dunkin's cameras do not cover this area of the parking lot. Traffic Bureau update (Officer Gaudet): On Monday, February 10, 2020 I made contact with the registered owner of MV2, Michael Ricci. Ricci stated he is currently a resident of San Diego, California and he was in Lake Tahoe over the past weekend. Ricci stated his vehicle (2009 Honda Accord, CA: 7XAS230) was with him at this time so he could not have been involved in this crash. Ricci's registration of MA: 2LD812 was cancelled as of 2/24/2017.

On 02/15/20 at approximately 13:27 Officer Anderson and I responded to #14 Elliot St- The Negoshians Sunoco Gas Station to meet with the operator of Mass. Reg# 7EK387 Grey 2017 Mazda 3, Galina Zavlin. Zalvin stated she spoke with Edward Negoshian who told her she ran into Fuel Pump #6 and dented the metallic base after striking the fuel nozzle of Pump #4 approximately a half hour ago while she was fueling up. Zalvin admitted to hitting Fuel Pump #4 with her passengers side mirror but denied ever striking Fuel Pump #6 or the Oil Dolly Display as stated by Negoshian. Officer Anderson asked Negoshian if his security cameras were working and he pointed to the closest one and said "not that one" . He went on to say that he didn't know how to retrieve the security footage. I observed the Mazda 3 in question and didn't see any damage or dents in the front of the vehicle. The approx 1-2" dent on Fuel Pump #6 is showing signs of rust and appears to be old damage. Employee John O'Donnell stated he saw Zalvin strike Fuel Nozzle #4 but never saw her hit Fuel Pump #6 with her vehicle.

I raised occupant of #23 and spoke to #2 operator who was not on scene by cell phone. #2 operator was unaware of any collision. #2 texted pictures of his Mv's rear bumper. One picture appeared to show a light colored scrape on driver's side upper rear end. Mv#1 is dark blue. Mv#2 is bronze. Both parties agreed to exchange information. Both parties were advised the matter would be documented and to proceed further with their respective insurance companies.

On Saturday 6/27/20 at approximatly 0700 hours while assigned to marked unit n499 I was dispatched to the parking lot of 188 Florence Street (Heathwood Rehab) for a report of a motor vehicle that backed into a pipe comng from the building. Upon arrival I observed the vehicle had backed from a parking spot at 188 Florence Street into the rear building of 160 Boylston Street striking the gas meter. I could hear and smell gas coming from the meter and immediately cleared the reporting party from the area and requested the assistance of Sgt. McNeil, Fire Dept, National Grid and Maintenence from the Avalon (160 Boylston Street). Fire arrived on scene and was able to slow the leak of the gas and I was able to speak with the reporting party identified as SANFORD Troy. Troy states that he is an employee at 188 Florence Street and got to work a little early this morning. He states that he stayed in his car awaiting the start of his shift and must have fallen asleep with his foot on the brakle and the transmission still in reverse. When he fell asleep he states his foot must have come off the brake causing him to back into the gas meter of 160 Boylston Street. The impact not only damaged the gas meter, but also did damage to the rear center of Troy's vehicle. Through my observations of Troy's demeanor and actions he did not appear to be impaired in any way. Troy stated that he did not get much sleep last night and was tired. He also stated that he did not need any medical attention Fire remained on location awaiting the arrival of National Grid. After interviewing Troy and confirming the area was secured I cleared the location without further incident

Owner of MV1 stated this morning at approximately 0830 hrs she observed damage on the rear end of her vehicle. It should be noted the owner of MV1 was not home the last couple days and observed the damage today. Owner stated she spoke to building management and was able to retrieve a video of MV2 colliding with her vehicle, which I was able to view. Owner stated MV2 which has an assigned parking spot #91 next to hers ( #89), and is seen on video on 07/25/2020 at 0942 hrs back up, attempt to turn and collide with the rear end of her vehicle. MV2 is then observed returning back to #91 at 0946 hrs. The operator and owner of MV2 is then observed exiting the vehicle and walking away. I was able to speak to owner and operator of MV2 who stated she took her vehicle for a spin around the parking lot that morning because she wanted to see if she could hear a noise coming from the vehicle that she heard the day before. Owner and Operator of MV2 stated she was not aware that she struck MV1. No injuries. MV1 sustained minor damage to the rear end. MV2 sustained minor damage to the right front end. No vehicles towed.

At approximately 1735HRs on Saturday, August 29, 2020, I was dispatched to 14 Elliot St. (Sunoco Gas Station) for a report of a two car MVA with one party refusing to exchange RMV information. Upon arrival, I spoke with the operator of MV1 (MA Pass: 6GT965), who stated that he was stationary in his vehicle at pump 3, when the operator of MV2 reversed into his vehicle after refueling at pump 5, which is directly in front of pump 3. The operator of MV2 got out of her vehicle and began to yell at him and refused to give him her RMV information, claiming he hit her. The operator of MV1 stated that he called the police and the operator of MV2 subsequently left the scene prior to my arrival. Officer Keefe (N495) attempted to raise the registered owner of MV2 (MA Pass: 2KWK11) at her residence with negative results and dispatch was unable to find a cell phone for her. I was able to speak with witness 1, who concurred with the operator of MV1's version of events. After speaking with witness 1, the operator of MV1 and viewing his video he recorded on his phone showing a party believed to be the owner/operator of MV2 entering her vehicle (Caucasian Female with the same light/strawberry blonde hair, as seen in her RMV photo), I have issued her MA Criminal Citation T2015042 for violating one count of MGL Ch. 90 S. 24. (Leaving The Scene Of Property Damage) and mailed it to her address. Note: the cameras at the gas station are not operational at this time and a copy of the aforementioned video has been forwarded to IT to be attached to this report.

On 9/1/20 while working N493, I responded with N497 and the Fire Dept to 40 Crescent St for a report of a car into the house. On arrival I observed MA 6CM450 in the backyard of #40 unit C. It was stationary against the large AC Compressor in the rear of the unit, and appeared to had run over several large shrubs before coming to a stop. I spoke with vehicle owner and operator Marion Cohen 8/27/46. Cohen stated that as she was parking in the rear lot of the complex, she accidentally mistook the brake for the gas pedal. She in turn ended up going over the curb and through some bushes before hitting the compressor. There was a passenger in the vehicle identified as Milton Kornfeld 3/10/42. Both occupants were checked out by medics and declined transport. The airbags did not deploy. There does not appear to be any structural damage to anything other than the compressor. NFD checked the integrity of the house, and the compressor was unplugged. I took pictures of the scene and submitted the memory card to IT. Homeowner Xiaozhi Zhao and his family were present inside at the time of the collision and are unharmed. Tody's responded and removed the vehicle from the property. It sustained heavy front end damage and was towed to their lot via flatbed. A m/v inventory form was filled out. This report number has been provided to all parties for their respective insurance companies.

On Wednesday, October 7th 2020 while in N526 I responded to 791 Walnut Street the Newton Cemetery Corp for a motor vehicle accident involving a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. The accident occurred inside the cemetery on the public way by the entrance. Upon arrival I spoke with the pedestrian identified as Tova Goldring. Tova stated she was walking in the roadway when she was struck by MV1. Tova stated MV1 was turning left onto the road that leads to the exit and when she realized the operator was not stopping she put out both her hands and made contact with the hood of MV1. Tova stated the impact caused her to fall to the ground and roll into a grassy area next to the road. Tova had minor scrapes from the accident and signed a patient refusal. The operator of MV1 identified as Antonio Domenico Ricci was on scene and his primary language was Italian. I was able to contact his daughter, Daniela Boccia who arrived on scene and translated Antonio's statements. Antonio stated he was turning left onto the main road that leads to the exit of the cemetery and had his left signal activated. Antonio states the sun was glaring into his eyes and made it hard for him to see. Antonio stated he took the left turn and never saw the pedestrian. Antonio states he heard a thud on his hood and stopped the vehicle and that is when he realized he had struck a pedestrian. Antonio was not injured and there was no damage to his vehicle. Pictures of the scene and Tova's injuries were taken by Officer Wilson and submitted to the IT Bureau. Daniela took possession of MV1 and drove Antonio to his residence.

On Wednesday, November 18th 2020, at approximately 3:40pm, I, Officer Brooks, responded to 624 Washington street, the Santander Bank, for an MVA in the parking lot. Upon my arrival I spoke with the operator of MV1 (MA reg 668EH9), who stated she was backing out of a parking space when she struck MV2(MA reg 27247 ) in the rear end. There was no visible accident damage to MV1. I spoke with the operator of MV2 who stated she was parked when her vehicle was backed into. The contact was to the rear of the vehicle, but the operator stated her car was now making noises and would not drive. I observed the front drivers side tire to be crooked from damage under the vehicle. The entire vehicle was covered in scratches and nothing distinguishable from an accident. The operator of MV2 stated her vehicle was struck hard, so I offered her medical attention which she declined. She contacted AAA to remove her vehicle. Both operators were provided this report number.

Operator of MV1, Emily Stoller, states she was operating MV1 in the parking lot at 200 Boylston Street when her friend, Alvin Day, asked her if he could drive MV1, which she gave him permission to do and was under the impression Mr. Day had a valid driver's license. It should be noted this area is regularly used to get to commercial businesses and has the right of access to the public. Ms. Stoller states Mr. Day was only driving about 15mph when he lost control of the vehicle and crashed head on into a light pole. MV1 sustained heavy front end damage and was towed from the scene by Tody's towing. Mr. Day states he does not have a valid driver's license and asked Ms. Stoller if he could drive MV1. Mr. Day states he was "goofing off" driving around the parking lot at approximately 15mph when he "cut the steering wheel too much", lost control of the vehicle and crashed MV1 head on into the light pole causing the driver's side airbag to deploy. Mr. Day states he lost consciousness for a brief moment where he was assisted out of the vehicle by the rear passenger, Alex Kakooza. Mr. Day was evaluated by the medics and transported to Children's Hospital. Ms. Stoller and Mr. Kakooza were also evaluated by the medics where they obtained patient refusals. All three juveniles had an adult family member respond to the scene. Ms. Stoller's father, Gregory Stoller, Mr. Day's mother, Quonekuia Day and Mr. Kakooza's sister, Latifah Kakooza. Professional electrical contractors responded for the damaged light pole and mall security will be notifying the property owner.

Mv#1 (tractor trailer) operator stated he had a clear access road in the parking lot as he was backing to make a delivery into the Whole Foods loading dock. #1 further stated before he backed up, he observed Mv#2 off to an adjacent side corner of the parking lot, which left the access road clear leading to the loading dock. #1 stated at that time while he was backing, #2 attempted to pass his trailer on the right. A collision occurred with the passenger side rear trailer and the driver's side front of #2. #2 operator stated he was attempting to exit the parking lot by the access road leading out to Beacon St. #2 further stated he observed the trailer backing up, at which time he stated he came to a complete stop and was honking his horn to alert #1. #2 stated at that time, #1 did not stop and backed into his driver's side front end. #2 sustained moderate damage. #1 sustained no damage to the rear trailer. Observations of Mv positioning on arrival was:the trailer was almost fully in the center of the access road leaving only about a 3 foot opening from the curb on its right. Mv#2 was in the far right lane of the access road. Although #2 reported he came to a complete stop prior to the colllision, it appeared #2 intended to pass and approached too closely to the rear of the trailer to leave enough safe distance between both Mvs.

On March 29, 2021 at approximately 17:40 hours I responded to 33 Boylston St, Juice Pressed, for a report of a single vehicle crash into a building. Upon arrival, I observed a black Mercedes SUV (MV1) parked in front of Juice Pressed with glass on the hood of the vehicle. The window pane store front and right door to the "Juice Press" storefront were shattered and broken. The operator of MV1 said that she was stopped in the parking space in front of Juice pressed ordering her drinks via the mobile app for pickup when she thought the vehicle was in park she took her foot off of the brake and accidentally hit the gas peddle. The vehicle crashed into the front of the store traveling about 2-3 feet into the building, breaking the front glass and door. No patrons were within striking distance of glass or the vehicle. Two employees were inside the business and said they were 15-20ft away from where the vehicle made contact. Newton Fire were on scene, management for "the Street", and "Juice Pressed" were notified and responded and arranged for the storefront to be boarded up. Management and vehicle owners were given the accident report number and information on how to obtain the report.

On 03/30/21 at 09:58 hours, Ofc. Marini (N537) and I (Ofc. Wilson (N523) responded to the parking lot of Tedeschi's located at 612 Washington Street for a two car motor vehicle accident involving a city motor vehicle which belonged to the Newton D.P.W.. At the time of the accident, weather conditions were clear and sunny with dry road conditions. The operator of M/V # 1, Danielle Finklea MA D/L S09835151, states she was eastbound on Washington Street in her 18' Mitsubishi Outlander (MA reg. # 9TM745) and turned right in to the parking lot of Tedeschi's. While pulling in, Finklea misjudged the distance that M/V # 2 was parked in a spot to the left of her. She then struck the right side of M/V # 2's rear bumper which sustained scrape marks to it. The Outlander sustained scrapes to the driver's side front quarter panel area and a broken headlight lens cover to that same side. The other vehicle (M/V # 2) was a 2014 Ford F350 pickup bearing MA Official Reg.# M87258 which is owned by the City of Newton Department of Public Works. At the time, the F350 was parked in a parking spot in front of the store unoccupied. The operator, Robert Mazzarella, stated he saw the F350 shake as it had been struck so he walked over to the other operator involved. No injuries reported. Ofc. Marini took digital photos of the scene and submitted them to the I.T. Bureau.

On Friday, April 2, 2021, I spoke with Mr. Michael Maguire from the City of Newton Buildings Department in the Traffic Bureau. Mr. Maguire wanted to report a crash the he was involved in this morning with his City of Newton owned vehicle. Mr. Maguire reported the crash occurred at 612 Washington Street, Newton. Mr. Maguire stated he was traveling (E) through the 7-11 convince store parking lot at approximately 0755 hours this morning. Mr. Maguire stated he was operating a City of Newton owned 2010 Ford Ecovan (MA MVN: M99243) at the time of the crash. Mr. Maguire stated at this time a vehicle to his right backed out of it's parking spot and it's rear driver side bumper area crashed into the middle passenger side of his vehicle. I observed minor damage to the middle passenger side door area of MV1. Mr. Maguire reported no injuries. Mr. Maguire reported MV2 was a 2015 Honda Civic (MA: 588LG8) and the vehicle was being operated by Mr. Donald Sampson (S29010431). Mr. Maguire stated Mr. Sampson exchange information with him and he observed damage to the rear driver side bumper area of MV2. Mr. Maguire provided me with a picture of the damage to MV2. On Sunday 4/11/2021, at approx 1219hrs, while assigned to N498, I responded to 141 Needham St in Newton for a MVA car into a building. There I observed MV1 in contact with the building used by retail store TJ MAXX. The operator, Beverly Linsky, was already outside the vehicle. I assisted Linsky to a bench where she was evaluated by Fallon medics. Linsky refused medical treatment. She stated that she was "fine". It should be noted that despite the heavy front end damage to MV1 the airbag did not deploy. Linsky states that while attempting to park her vehicle she accidently stepped onto the gas pedal instead of the brakes and drove into the TJ MAXX store building. MV1 was towed from the scene by Tody's towing service. The store manager was notified. On Wednesday, June 16, 2021 while assigned to Traffic unit N525, I responded to the front entrance of the Brown Middle School located at 125 Meadowbrook Road in Newton for a single vehicle crash involving a City of Newton contracted school bus. It was also reported by Newton Dispatch that one of the occupants on the bus had suffered an arm injury as a result of the crash. Meadowbrook Road is a public way maintained by the City of Newton. The road surface at the time of the crash was dry. The weather at the time of the crash was clear and sunny. When I arrived, I was updated by Officer Iarossi that the crash involved a City of Newton contracted school bus transporting students from the Williams School (141 Grove Street, Newton). The students were participating in "Step Up" day and were leaving Brown to go back to Williams. Officer Iarossi stated a student on the bus was injured as a result of the crash. Tegan Quirk was transported by Fallon Medics to Newton Wellesley Hospital with a right arm injury. The remaining students that were on the bus reported no injuries to staff at the time and were moved to another bus to be transported back to the Williams School. I asked the Brown staff that was assembled in front of the school to contact the Williams School and ask them to hold the children involved in the crash at the school once they arrived. I made this request so we could make sure there weren't any new injuries reported; and I wanted to inquire if any of the children witnessed the incident. The staff stated they would contact Williams and relay the request. I spoke with the operator of the school bus (MA BUN: BU44536), Mr. John Aubert (S48613114). Mr. Aubert stated he was operating bus #361 while transporting students from Brown Middle School to the Williams School. Mr. Aubert stated as he was preparing to leave Brown, he made an announcement to students on the bus to sit down. Mr. Aubert stated he got the okay to leave from one of the staff members on the bus. Mr. Aubert stated there was another bus parked in front of him prior to leaving. Mr. Aubert stated he began to reverse the bus to create distance from the bus in front of him, and then operated the bus forward to exit from his position on the right side of the road and merge into the travel lane to his left around the bus parked in front of him. Mr. Aubert stated as he merged onto the roadway, he heard screaming from the students on the bus and the rear passenger side of the bus made contact with a brick support column in front of the school positioned along the sidewalks edge next to the roadway. Mr. Aubert stated he exited the bus and observed a student's arm pinned on the rear passenger side of the bus between the side of the bus and the brick column. Mr. Aubert stated he got back in the bus and reversed it until the students arm was free. Mr. Aubert stated at this time, staff helped the injured student. The staff there helped facilitate transportation for the remaining students back to the Williams School. Mr. Aubert reported no injuries, but was concerned for the welfare of the student. After speaking with Mr. Aubert, I observed damage to the school bus and the brick support column as a result of the crash. I observed fresh damage (scrapes) to the rear passenger side of the school bus between the last passenger side window and the back of the bus. I also observed fresh damage to the brick support column. The column is a support to an overhead covering that runs from the sidewalk to the front entrance of the school. There was fresh paint transfer from the bus and a crack in the column. Pictures were taken of the damage and submitted to the IT Bureau. The travel lane where the crash took place is a one way access road that runs from Meadowbrook Road to the front of the school, and exits onto a parking lot between the Brown Middle School and the Oak Hill Middle School. I responded to the Williams School to follow up with staff there regarding this crash. All of the students that were involved in the crash had been released prior to my arrival. I spoke with the Principal of the Williams School, Ms. Ayesha Farag, Ms. Michal Shapiro (Social Worker), and Ms. Katie Costello (School Nurse) about the students on the bus. They stated no injuries were reported to them, and they notified the parents of the involved parties about the crash. They also provided me with contact information for all of the students on the bus at the time of the crash. I spoke with the three teachers that were on the bus at the time of the crash, Ms. Kimberly Andersen, Ms. Tess McLean, and Ms. Katherine O'Leary. All three teachers stated prior to the bus leaving Brown Middle School they accounted for all of the students and were ready to leave to travel back to the Williams School. One of the teachers recalled hearing the bus driver make an announcement to the bus that they were about to leave, but could not recall the exact wording of the announcement. The teachers stated as the bus began to move, they heard screaming from the rear of the bus, looked back, and saw a student's arm stuck outside the window. After realizing the student was injured, they assisted with helping to free the student from his pinned position. They helped the injured student until EMS arrived on scene. They assisted the remaining students by directing them away from the crash and onto another bus for transportation back to Williams. The three teachers reported no injuries as a result of the crash. I responded to the Newton Wellesley Hospital Pediatric Emergency Room to check on the status of the injured student, Tegan Quirk. I spoke with Tegan and his mother, Kathryn Quirk in his room. While attempting to talk to Tegan and his mother, Tegan became very upset and did not want to talk about the specifics of the crash. Out of courtesy for Tegan, I exited the room and spoke with his mother outside the room. I explained to Ms. Quirk the events of the crash, and how her son's arm became injured. I asked Ms. Quirk if she could assist me with faciliating a conversation with Tegan about the events of the crash. I explained to her I understood how traumatic the events of the crash were for her son, but as part of my investigation I would need to find out what led to Tegan's arm being outside the bus window at the time of the crash. While speaking to his mother, Tegan explained that he was waving to a friend with his arm out of the bus window when the bus started to move. Tegan stated his arm hit the column, and became stuck between the bus and the column. Tegan stated he was screaming for the bus to move and that it took forever for the bus to move so he could free his arm. Tegan stated the bus driver gave no warning to the students that the bus was going to start moving. The staff at Newton Wellesley Hospital stated Tegan has a right arm injury. The severity of the injury is unknown at this time due to further examination that needs to take place. I advised Ms. Quirk about the conversations I had with the other parties involved. I spoke with Mr. David Stickney (Head of Facilities) from the Newton Public Schools to try and obtain footage of the crash. Mr. Stickney stated unfortunately there is not footage of the crash from the Brown Middle School or Oak Hill Middle School surveillance cameras. I advised Ms. Regina Moody (Newton Public Schools Transportation Director) about the crash. Ms. Moody was able to show me footage of the incident from the camera inside the bus. In the footage, the bus driver can be heard announcing to the students prior to the bus moving, "Guys, guys, get off the seat and sit correctly". One of the teachers can be seen doing a count of the students. The teacher is heard saying to the student, "Everyone needs to be in a seat and sitting at 3 and 2 and 1 and thank you." Student are observed still standing up. The teacher is then heard saying, "sit down sit down." After the teacher passes the students in the back of the bus, she says, "We are good." to the front of the bus. Students are then observed standing after she passes. As the bus begins to back up, students can be observed standing in the rear of the bus and not sitting down on their seats. As the bus moves forward, a student (Tegan Quirk) can be seen standing on his seat and extending his right arm out of the last window on the passenger side of the bus. Screaming can then be heard as his arm makes contact with a support column on the passenger side of the bus. The bus is stopped, the driver exits the bus to asses what happend, gets back on the bus, and reverses the bus a short distance. The injured students right arm is released and he brings his arm back into the bus through the window. The amount of time between the students arm being pinned against the column to when it is released is approximately 40 seconds. After speaking with all parties involved and reviewing the footage from inside the bus, it appears that Bus #361 was pulling out of it's parked position in front of the Brown Middle School to enter the roadway and return students to the Williams School. A student that was sitting in the last row on the passenger side of the bus extended his right arm out of a window next to his seat to say goodbye to a friend. While his right arm was extended out of the rear passenger side window of the bus , the bus collided with a brick support column affixed to the sidewalk in front of the school. When the bus made contact with the column, the students right arm made contact with a brick support column and became pinned between the column and the passegner side of the bus. After the students arm was freed from it's pinned position, the student was able to bring his arm back into the school bus and was transported to Newton Wellesley Hospital for a right arm injury.

Mv#1 was backing out of a stall in the Trader Joe's parking lot located at 1121 Washington Street when it scratched Mv#2 bumper. The collision was reported by OpMv#2 at the Newton Police Department. -OpMV#2 reported that she parked MV#2 in a stall in the parking lot and that upon returning she was informed by a witness her vehicle had been struck and the operator of the vehicle left the area. OpMv#2 was provided with both the registration plate and description of the other vehicle. -I conducted an inspection of the damage to MV#2. I observed there was orange paint transfer on the vehicle's rear bumper, surface scratches; however, I did not observe structural damage of the bumper. -I next made contact with the OpMv#2 by sending Melrose police to his residence. OpMv#1 stated he had shopped at the Trader Joe's and was not aware he had struck another vehicle until the damage was pointed out to him by the Melrose officer. -There were no injuries reported at the time of this report and neither vehicle was towed from the scene.

On Monday, July 12th 2021, at approximately 5:24pm, I, Officer Brooks, responded to the parking lot of the Barn, 229 Walnut street, for a car accident. I spoke with the owner of MV1(MA REG 9937PH), who stated she had been parked here all day, and when she came out to her car there was a note on the windshield stating "Sorry i hit your car, foot slipped on wet". Her vehicle sustained damage to the drivers side front bumper. The note contained the license plate for MV2 (MA REG 54542), and the registered owners name. Dispatch attempted to reach her by phone but was unable to speak with her. MV2 was still on scene and parked in front of MV1. MV2 had minor damage to the front bumper. It appears the operator of MV2 was pulling into the parking spot and struck MV1. There were no injuries and both vehicles were able to be driven from the scene. The report number was given to both involved parties.

On Wednesday 9/15/2021 at approx 1045hrs, while assigned to the cell block, I took a past MVA report at the front desk. There I was met by Michael Krongel who states that at approx 1005hrs this morning his parked vehicle(MA reg1XJV53) was hit by another car at the Newton Wellesley Hospital parking garage. A witness stood by and gave Krongel their info along with a license plate number for the vehicle that struck his. The rear driver side of his vehicle was damaged. The second vehicle came back to a 2020 Ford Fusion MA reg 1LZR85 owned by Breanna Wasson DOB 4/23/1993. See witness statement. Newton Dispatch was able to find a number for Wasson on record. I called the number and left a message on its voicemail. It it not known at this time if Wasson was operating MV2. Traffic Bureau update (Officer Gaudet): On Thursday, September 16, 2021, I attempted to make contact with the registered owner of MV2, Ms. Breann Wasson, with a negative result. A message was left for her to contact me. A "Hit and Run" inquiry was mailed to Ms. Breann's address listed with the registry. On Thurday, September 23, 2021, I spoke with Ms. Wasson via telephone. Ms. Wasson acknowledged she was involved in the crash and had to leave with out immediately exchanging information because she was late for an appointment. Ms. Wasson stated when she returned to her vehicle, Mr. Krongel had left his business card on her winshield. Ms. Wasson stated she contacted Mr. Krongel to exchange information but he was not interested in speaking with her. Ms. Wasson stated she contacted her insurance company and reported the crash to them. Ms. Wasson did not provide information about any damage to her vehicle. Due to Ms. Wasson admitting fault for the crash and reporting the crash to her insurance company, no further action will be taken.

On September 9, 2021 at approximately 10:56 hours I,Officer Guarino, responded to 79 Florence St for a report of a single vehicle crash into a gate and pole. Upon arrival, I observed MV1 with heavy front end damage crashed against a brick structure that holds the iron gate of the property. The gate was detached from the structure and on the sidewalk. The operator of MV1 told me that she was driving northbound towards Florence St when she observed a UPS truck entering the property. Operator of MV1 thought the UPS truck entered her lane of travel and swerved to the right which caused her to crash into the gate and brick structure. The operator of the UPS truck was on scene and had no damage on his vehicle. Multiple witnesses observed the crash who said that the UPS truck was traveling slow and stayed within its lane of travel. A witness said that the operator of MV1 was "flying" through the parking lot and swerved into the gate and structure. I observed the video footage at 79 Florence St where you can see MV1 traveling through the parking lot and it appeared that the UPS truck had already entered the property when MV1 took the corner and MV1 drove into the gate and Brick structure. I emailed the property manager to have the footage emailed to me, so I can attach it to this report. Witnesses, Rich (781)351-0172 and Mario (615) 981-4890 left their information with the UPS driver but I was unable to make contact. The witness I spoke with on scene wanted to remain anonymous.

On 10/7/21 at about 1145am I was dispatched to 1101 Beacon St the Walgreen's parking lot for an MVA with injuries. On arrival, I observed 3 parked unoccupied MVs heavily damaged that were smashed into each other. I was approached by one of the MV owners, Tony Dooley. Mr Dooley stated he was walking out of the Walgreen's front door heading to his parked MV (#3) which was approximately <30 feet away. His attention was suddenly drawn to a speeding MV (MA reg 3MKH84) driving through the lot that veered right and smashed into the 3 cars. Mr Dooley's MV was the middle car smashed between the other 2. The female operator briefly stopped afterwards and pulled into an adjacent parking spot at a crooked angle. I approached the female operator who was being taken out of her car and placed onto the stretcher by medics for transport. Female operator was identified as Ms Norma Greenberg. I asked her what happened? She replied in a bewildered manner "I have no idea." I asked her if she was taking any medications today and she replied yes, to treat a spinal condition she has. Ms Greenberg had lacerations on her legs and arm with heavy bleeding as a result of the impact.Ms Greenberg's vehicle (#1) was towed with significant front damage and airbag deployment. MV#2, the first car struck on impact, was towed with very heavy passenger side, rear end damage and driver's side damage. MV#3, the middle vehicle, was towed with significant passenger side damage, flat tire and driver's side damage. MV#4, sustained significant passenger side damage which was able to be driven from the scene. Based on observations and statements, it appeared Ms Greenberg had suddenly dangerously accelerated through a congested parking lot causing heavy damage. Ms Greenberg's driving history shows surchargable accidents from 7/1/21, 11/1/20, 8/25/15 and 12/26/10. Due to Ms Greenberg's inability to recall how or what happened, I am seeking an Immediate Threat application due to and not limited to medical reasons to suspend Ms Greenberg's license through the RMV.

On October 26th 2021 while assigned to the front desk, Hanna Yoon DOB 9/9/83 came in to report a minor past mva/pedestrian. Yoon states that around 0820 hrs today, she was pulling out her driveway at 357 Langley Rd (Ma reg 5HL884). Yoon did not see a pedestrian approach from the right, and contact was made at a slow rate of speed. Yoon states the pedestrian did not fall down. She believes that the front right part of the vehicle hit the person's left thigh. Yoon asked the pedestrian if she was ok, to which the woman stated she was fine. The woman asked for and was provided Yoon's cell phone number before walking away. Her clothing description is unknown. Around 1100hrs Yoon then received a text from 617-913-5141. The person texting identified them self as the woman who was hit, and stated she was in the hospital with leg pain. She then texted that the hospital would contact Yoon for more information. Yoon did not respond and has not been contacted by anyone since this text. A name was not provided by the texter. There are no entries in QED indicating that the unknown pedestrian has called to report this accident. I tried to call the number provided with negative results. At this time, there is no identifiable victim. Yoon was advised that I will document her statement, and in the future it is best to call police to the scene of a crash in order to avoid confusion. Update by Lt. Dangelo: On 10/27/21 at approximately 0830 hrs. the pedestrian from the accident, Woo Jin Seo came into HQ with her mother to report the accident. She stated that on 10/26/21 at approximately 0820 hrs. she was walking to Newton South High School from 383 Langley rd. when a vehicle backing out of a nearby driveway bumped her. She stated that she did not fall down, but did suffer an injury to her leg. She stated that the operator of the vehicle stopped and checked and left her phone number. Ms. Seo went to Beth Israel Lahey urgent care the same day for leg and shoulder pain. I contacted the operator Hanna Yoon and updated her.

On November 21, 2021, at 17:07 hours Officer Helms and I, Officer Joyce, were dispatched to a report of a motor vehicle crash in the Trader Joe's parking lot at 1121 Washington St. When I arrived on scene I was approached by a male, later identified as Edward Plotkin, he stated he had struck the parked car but the female in MV2, later identified as Anneline Smal, would not exchange papers. On Tuesday 6/14/2022, at approx 1311hrs, while assigned to N496, I responded to the parking lot of 1280 Centre St in Newton for a MVA hit&run with a parked vehicle. There I observed fresh damage to the rear bumper of MV1. The owner of MV1 was notified of the accident and was given a report number. I was unable to locate the owner of MV2, Song Mai. Mai was cited for leaving the scene of property damage. MA citation number T2015014 was mailed to the address listed on Mai's registration. It should be noted that Mai has an extensive drivers history including leaving the scene of property damage.

Operator of MV1 was parking in a parking spot outside of Pressed Cafe located at 300 Needham St. Operator believed she was in Park but was actually in Drive. Operator mistakenly pressed on the gas and accelerated over the curb and through a large window and ended up inside of Pressed Cafe. Operator and her daughter, Brynn Bryant, were trapped in the vehicle and freed by NFD. Operator and Brynn were transported to Newton Wellesley Hospital. The other 2 passengers, Gabe Cohen and Marc Andrew, were not trapped and declined any medical attention and left the scene after speaking with the medics. MV1 was towed by Todys. Nobody inside Pressed Cafe was injured. John Faggiano was standing next to where the car went through the building but declined any medical attention. Pictures and a video of the accident from the security system was sent to IT to attach to the report.

On Friday 6/24/2022 at approx 0951hrs, while assigned to N496, I responded to the rear parking lot of 718 Beacon St in Newton. There, I spoke with the operator of MV1 who stated that her vehicle was unoccupied and in a parking space when MV2 ( USPS Truck #7533313) struck the front drivers side of her vehicle. Operator of MV2 states that he attempted to pull out of his parking space but accidently hit MV1. MV1 had considerable damage to the front drivers side corner of the vehicle. MV2 had minor scrape damage to its passenger side.

Owner states sometime between the evening of July 16, 2022 and the morning of July 17, 2022, his vehicle was parked unoccupied in the parking lot unoccupied next to his residence and was struck on it's drive side by an unknown vehicle. I observed damage to the rear passenger side door/fender area. No cameras are present in the area. No vehicles parked in the lost appeared to have fresh damage. Owner of vehicle was advised.
