SHE GAVE THE OWNER OF VEHICLE #1. I RAN THE PLATE AND IT CAME BACK TO A BLACK JEEP OWNED BY ( ANAPAULA GUERES ). ATTEMPTS TO CONTACT VEHICLE #2 WAS UNSUCCESSFUL. VEHICLE #1 STATES SHE HAD NO INJURIES. ADVISED TO CONTACT HER INSURANCE COMPANY. ON 1-24-19 AT APPROX. 0930HRS. WHILE WORKING N492 I WAS INFORMED BY CIVILIAN EMPLOYEE ( STACY ANNESE ) THAT THE OWNER OF VEHICLE #1 ( AYENG FONG ) HAD COME TO THE STATION TO INFORM ME THAT THE ORIGINAL LATE NUMBER SHE HAD GIVEN ME WAS WRONG. SHE STATE THE PLATE NUMBER OF THE VEHICLE THAT ALLEGEDLY HIT HER CAR WAS ( MA REG: 9TK518 ). A REGISTRY CHECK CAME BACK TO A 2019 BLACK JEEP CHEROKEE OWNED BY CHRISTINE MITCHELL OF 169 ADAMS ST. I WAS UNABLE TO MAKE CONTACT WITH MS MITCHELL AND WILL ATTEMPT O DO SO ON NEXT SHIFT. Traffic Bureau update (Officer Gaudet): I attempted to contact the owner of MV2, Anapaula Guedes (S28004541) with a negative result. A Hit and Run inquiry was mailed to the Guedes.

MA commercial reg M94754 (vehicle #1), a City of Newton pickup/sander truck, was located crashed over the curb in front of 1970 Beacon St. The vehicle was unoccupied, had the keys in the ignition, and was reported stolen just a few minutes prior to discovery. The reporting party of the stolen truck and uninvolved occupant is identified as City DPW worker George Sementelli. The witness/passerby who discovered the collision is also a DPW worker, identified as Donny Brandt. Pursuant to an investigation outlined in incident #19002213, the operator at the time of the crash was determined to be Michael Carter. It is believed that Carter stole the vehicle from 295 Auburn St, fled the area, and ultimately crashed in front of 1970 Beacon St. Corroborating facts outlined in the incident report put Carter as the operator. The vehicle itself was located partially on the sidewalk and partially on the road, therefore Carter is being cited with MGL 89/4A - Marked Lanes Violation (T1268423). Due to the fact it is believed that Carter fled the scene after the crash, MGL 90/24/C - Leaving the Scene of Property Damage was added as well as MGL 90/23/D Operating After a Suspended LIcense, and MGL 90/24/O - Reckless Operation of a Motor Vehicle. The crash occurred during busy weekday commuting about 0.7 miles from Angier Elementary School. The last 3 charges mentioned have been listed on a second citation (T1268422). Officer Brooks also issued T0648082 for OUI Drugs MGL 90/24/G. See the incident report for details about this charge. In total, there are (3) citations issued to Carter. The vehicle sustained heavy front end damage. It should be noted that the utility pole a couple of houses down from the crash site (Beacon/Varick Rd, pole #212) sustained fresh, minor superficial damage, consistent with the vehicle crashing into it before verring onto the sidewalk. Pictures have been taken and submitted to the IT bureau. Tody's responded and towed the vehicle to the police garage for processing due to the pending other criminal charges involved in this accident/incident. Michael Carter was subsequently placed under arrest and his citations have been placed into his property. Carter suffered a hand injury due to the crash an was transported to NWH Hospital for an evaluation after he was booked.

On 1/19/19 I responded to 25 Normandy Rd for a report of a past hit and run. I spoke with the Operator of MV#1 who stated the accident had taken place in front of Tom's Pizza, 423 Lexington St, at approximately 1830 hrs. Operator of MV#1 stated she parked her vehicle in a parking spot directly in front of the restaurant and went inside. When she returned approximately 5 minutes later she realized her vehicle had been rear ended. Two witnesses provided her with information about the accident and the plate number. (See witness narratives.) MV#1 suffered moderate damage to the rear and was driven from the scene. Waltham PD responded to the address listed for MV#2 and were not able to locate the vehicle. At approximately 2000 hrs, I was contacted at the police station by the owner of MV#2's wife. After further investigation (see incident report #19002425) it was determined that the operator of MV#2 at the time of the accident was the vehicle owner's brother. I spoke with Operator of MV#2 on the phone and he stated that the road was icy and slippery due to snow and he crashed into the back of MV#1. He stated he was nervous and thought he had only hit the sidewalk curb. Operator of MV#2 was issued citation T1440626 by mail for 90/24C Leaving the Scene of Property Damage, 90/10A Unlicensed Operation and 90/24 Use of MV Without Authority. He stated the vehicle suffered minor damage to the front.

On the above date and time I responded to the area of St.James St at Washington Street for a report of a past hit/run with City traffic signal knocked over. Upon arrival I observed the the traffic signal on St. James Street westbound just before the Washington Street bridge on the roadway. I did observe what appeared to be a sidewalk plow tire impressions on the sidewalk to the traffic signal. I cnanvassed the area with negative results and I am waiting to hear back from Public works who may have been assigned the area to plow the side walks. I also checked for any cameras in the area with negative results. I took pictures of the crash site area and I will have them downloaded at th I.T. Bureau.

On 01/22/19 at approximately 14:25 I responded to Winchester St near Curtis St. for a report of a hit and run accident. Upon arrival I spoke with the operator of veh #1 who stated he was driving due West bound on Winchester St when a truck (White 3/4 Ton Pick up with plow in front and sander in back) turned left onto Winchester St from Boylston St. Veh #2 caught his plow on the drivers side rear passenger door and rear quarter panel of veh #1. The crash pushed veh #1 into the opposite lane for a moment until he could turn back onto his side. The operator of veh #2 pulled over and got out and said "you didn't see me?" and got back into his truck and took off down Winchester St West bound. The operator of veh #2 is described as a Hispanic male 18-25yrs old, 5-8'-5'11", lean build, clean shaven, yellow jacket with black hair. No license plate was ever obtained. Veh #1 was towed by Todys Towing and owner of veh #1 was given a ride home to Needham.

The operator of MV#1 stated that she was backing out of a parking spot in the Star Market Parking lot, #2040 Commonwealth Ave when she was involved in a MV crash. Operator #1 stated that she and the other operator both exited their MV's and even though there was obvious damage to MV#1, the operator of MV#2 refused to provide any information, got back into his MV and left the area. Operator #1 describes MV#2 as a large yellow backhoe that did not display a MV plate and she didn't observe a company name anywhere on the MV. The operator id described as W/M, approx. 40 YOA with brown hair. There is a backhoe type MV presently in the Star Market parking lot and operator #1 stated that is not the other MV involved. I also spoke with a Star Market manager who will view their exterior video footage of the parking area and notify me if it provides any information.

On January 25, 2019 at approximately 17:46 I responded to a motor vehicle hit and run. It was reported that a white pick-up truck crashed into a parked car and left the scene. I checked the area and was unable to find a vehicle matching the description. I responded to the crash location and spoke with the owner of MV1, who was not with his vehicle when it was struck. MV1 was parked legally on the shoulder of by 1743 Beacon St. The vehicle that struck MV1 left the travel lane and struck MV1 in the driver's side rear, causing damage. I spoke with the reporting party, Tim Leary, who heard the crash and saw a white Ford F-150 leaving the scene with damage on front passenger side. Leary did not see the license plate of the truck but thinks it was the 2006-2012 body style. The owner of MV1 was going to arrange for a private tow at a later time.

On 01/28/2019 Karen Griffin came into NPD Headquarters to report the following. Griffin states that on 01/17/2019 her vehicle (V1) was parked in a parking space in the lower parking lot of Wegman's 200 Boylston Street. Griffin states that V1 was backed into a parking stall near the entrance of the parking garage. Griffin states that between 1015 and 1130hrs V1 was stuck by an unknown motor vehicle (V2). V2 left the scene without leaving leaving their information. At the time of the collision V1 was parked and unoccupied. As a result of the impact V1 sustained minor damage to the passenger side front bumper. Griffin states that an employee of Wegman's witnessed the crash and described V2 as a Gray Honda Accord with a possible Maryland Reg JNK824. I conducted an RMV check of V@ registration with negative results. At this time the identity of V2 is unknown. No injuries were reported. Griffin stated she did not report the crash to the police until instructed to do so by her insurance company.

OWNER STATES HIS VEHICLE WAS PARKED ALL DAY BECAUSE HE IS DOING WORK AT THAT LOCATION. COWORKER GOT THE PLATE OF THE VEHICLE THAT HIT HIS VEHICLE. OWNER REPORTS NO INJURY AND WAS ADVISED O CONTACT HIS INSURANCE COMPANY. ATTEMPTS TO MAKE CONTACT WITH THE OWNER OF VEHICLE #2 WAS UNSUCCESSFUL AT THIS TIME. FOUND A SECOND NUMBER ( 860-301-1336 ) FOR THE METROWEST PACKAGING COMPANY AND LEFT ANOTHER VOICE MAIL.WILL TRY TO MAKE CONTACT AGAIN BEFORE PURSUING OTHER OPTIONS. Traffic Bureau Update (Officer Gaudet): On Monday, February 4, 2019 I made contact with the President of Metrowest Packaging, Brian Hersh. Hersh stated his company does fulfillment deliveries for Amazon. Hersh was advised that a vehicle registered to his company was allegedly involved in a hit and run accident. Hersh was provided information on the vehicle involved. Hersh stated an employee named Alexis was operating MA CON:T65665 on the date of the accident and provided a phone number for this party. On Tuesday, February 5, 2019 I spoke with Alexis Laporte-Soto (S84358654). Laporte-Soto stated he was operating a 2017 Mercedes Sprint van (MA CON:T65665) in Newton for Metrowest Packaging on the date of the accident. I explained to Laporte-Soto the vehicle he was operating was alleged to have been involved in a hit and run accident on Highland Street in Newton. Highland Street is a public way maintained by the City of Newton. Laporte-Soto stated the vehicle he was operating hit the driver door mirror of a parked card on Highland Street. Laporte-Soto reported the accident as mirror (MV2 passenger side) to mirror (MV1 driver side). Laporte-Soto stated after his passenger side door mirror made contact with MV1's driver side door mirror, he stopped his vehicle 15 feet down the road. Laporte-Soto stated he walked up to the car he hit and saw the mirror was bent back but there was no damage. Laporte-Soto stated he waited near the car for 10 minutes and the owner never showed up. Laporte-Soto also stated a "random guy" took a picture of his license plate. Laporte-Soto stated he said "hi" to this party but the party ignored him and left the area. I asked Laporte-Soto why he did not call Newton Police to assist him in finding the owner of the vehicle he hit. Laporte-Soto stated he didn't know. I also advised Laporte-Soto that the name of the owner of the vehicle he hit was printed on the driver side door next. The owner's phone number and email address were also printed on the driver door. Laporte-Soto stated he didn't see that. I spoke with the owner of MV1, Jason Correria. Correria stated a co-worker observed the entire accident and observed MV2's passenger side mirror hit MV1's driver side mirror. Correria stated his co-worker then observed MV2 travel down Highland Street with out stopping and pull into the Santander parking lot located at 1314 Washington Street. Correria stated his co-worker saw the operator of MV2 exit his vehicle, check his passenger side door mirror for damage, and leave the area. Laporte-Soto will be mailled Massachusetts Uniform Citation T1443121 for Chapter 90, Section 24 (Leaving the scene of property damage).

On Sunday, February 3rd 2019, at approximately 11:45pm, I, Officer Brooks, responded to a single car MVA at the intersection of River street and Auburndale avenue. Upon arrival I observed MV1(MA REG 7TT558) in the travel lane with both tires broken free from the axle. I spoke with the operator who could not recall how the accident happened, or what was struck. After an investigation the operator was placed under arrest for 90/24 OUI Alcohol. Officer Boyle was able to assist in the accident investigation. It appears the operator had been traveling westbound on River street and struck a utility pole on the drivers side front tire. The vehicle continued to the intersection of River street and Auburndale avenue and struck a curb here on the passenger side tire. Both tires were broken free from the axles. The vehicle was towed by Tody's and an inventory form was filed. Eversource was notified about the pole.

OPERATOR AND PASSENGER OF MV 1 STATED THAT MV 2 NEVER STOPPED TO SEE IF THEY WERE INJURED OR EXCHANGE INFORMATION AND CONTINUED ON CRAFTS ST EVENTUALLY TURNING RIGHT ONTO CALIFORNIA ST. MV 2 COULD ONLY BE DESCRIBED AS A WHITE COMCAST VAN WITH A LADDER ON THE ROOF. THERE WAS ALSO A WITNESS TO THE HIT AND RUN ACCIDENT IDENTIFIED AS RYAN WALSH WHO STATED HE WAS TRAVELING SOUTHBOUND ON CRAFTS ST WHEN MV 2 FAILED TO STOP AT THE STOP SIGN, CAUSING HIM TO SWERVE OUT OF THE WAY TO AVOID A COLLISION. WALSH ALSO STATED THAT THE VEHICLE WHO BLEW THE STOP SIGN AND CAUSED THE ACCIDENT WAS A COMCAST VEHICLE. I CONTACTED COMCAST SUPERVISOR BRIAN DELANO (617 756 6293) WHO WORKS OUT OF THE NATICK OFFICE AND IS IN CHARGE OF EMPLOYEES IN NEWTON. HE STATED HE LOOKED AT GPS RECORDS AND STATED ONLY ONE OF HIS EMPLOYEES WAS IN THE AREA AT THE TIME OF THE CRASH AND HE WAS STATIONARY AT ADDRESS ON MELVILLE AVE. I ALSO CALLED JOHN MCGUINNESS WHO IS THE SUPERVISOR FOR COMCAST WORKING IN WESTON, WALTHAM AND WATERTOWN AREAS. I LEFT A VOICEMAIL FOR HIM TO CALL ME BACK. I RECEIVED A CALL BACK FROM JOHN MCGUINNESS WHO RELAYED TO ME THAT HE SENT A MESSAGE OUT TO ALL OF HIS EMPLOYEES INQUIRING ABOUT THE ACCIDENT IN QUESTION AND ONE OF HIS DRIVERS CALLED HIM AND TOLD HIM THAT HE WAS THE PERSON INVOLVED IN THE ACCIDENT. I MET WITH HIM WHO WAS IDENTIFIED AS CAMDEN DUQUETTE AND HE STATED THAT HE STOPPED AT THE STOP SIGN, PROCEEDED FORWARD AND DIDN'T SEE A TRUCK COMING ON HIS LEFT. HE STATED HE WAS ABLE TO AVOID THE FIRST CAR AND DIDN'T REALIZE HE STRUCK ANOTHER VEHICLE (STATED HE THOUGHT HE STRUCK THE CURB).THERE WAS DAMAGE ON HIS PASSENGER SIDE CONSISTENT WITH STRIKING A VEHICLE AND NOT THE CURB. AS A RESULT OF THE INVESTIGATION HE WAS ISSUED MA UNIFORM CITATION T1441118 FOR CH .90 S. 24 LEAVING SCENE OF AN ACCIDENT (PROPERTY DAMAGE) AND CH. 89 S. 9 FTC STOP SIGN.

On arrival, I spoke to Matthew Digregorio DOB7/2/74 ( hereby referred to as operator #1) who was standing on the sidewalk next to MA reg 134TR9 (vehicle #1 ). Vehicle #1 had significant damage to the front and right quarter panels. It was straddled between the travel lanes and did not have the keys in the ignition. It was immediately apparent that operator #1 suffered an injury to his head, as there was a moderate amount of blood and small pieces of glass stuck in his forehead area. I observed operator #1 to be unsteady on his feet, have bloodshot eyes and slurred speech. I could detect a moderate odor of alchoholic beverage coming from operator #1 as I was approaching him, which was apparent from about an arms length away (it should be noted that medics were already en route to the scene at this time). Prior to the medics arrival, I asked operator #1 for a brief synopsis of what transpired. Operator #1 stated that he had just left Paddy's Pub and was driving westbound on Hunter St (which is approximately 1/8 mile from our location) when he collided head on with vehicle #2. Vehicle #2 was not immediately visible upon police arrival, however was pointed out as being located at the intersection of Hunter and Putnam St. I asked operator #1 where he was heading, as Hunter St is a back road not directly in the vicinity of Paddy's. Operator #1 could not answer the question and also remained silent when I asked him how much alcohol he had consumed this evening. At this time the medics arrived on scene. I asked operator #1 for the car keys to reliquish to the tow truck operator, which is standard operating practice. Operator #1 reached into his pocket and then provided me with the keys. Due to concern over operator #1's obvious head trauma, he was then transported to NWH to be evaluated while the investigation continued on scene. Two other parties approached myself and Officer Selig. One was the operator of vehicle #2, MA reg 1RD999. Operator #2 is identified as Luke Ferdinands DOB 5/11/80. The other party who approached is identified as Douglas Dasilva-Pimenta DOB 6/2/86 (Dasilva-Pimenta will now be referred to as operator #3). Operator #2 states that he was taking a right onto Hunter St from Putnam St, when he observed vehicle #1 on the wrong side of the road approaching him head on. Operator #2 states vehicle #1 then collided head on with him, took a right onto Putnam St, and continued onto the Washington St overpass near 1354 Washington St. Operator #3 states he was inside of #5 Hunter St when he heard a loud crash coming from outside. He looked out the window and saw vehicle #1 traveling at a high rate of speed on Hunter St towards Putnam St. Operator #3 observed damage to the right side of his parked vehicle (now referred to as vehicle #3) MA reg 7CAK80. He immediately got in his vehicle and proceeded to follow vehicle #1, which then crashed into vehicle #2 head on. Operator #1 did not stop until it was obvious that operator #3 was following close behind. This ultimately landed vehicle #1 at it's final location near 1354 Washington St. Vehicle #1 and vehicle #2 were not driveable and were towed to Tody's. It should be noted that there was a passenger in vehicle #2 at the time of the collision. Airbags were deployed and medics checked out both operator and passenger. Both declined transport. The passenger is identified as Elizabeth Ferdinands DOB 5/22/53.Ferdinands' version of events coincide with that of operator #2. Upon speaking with Officer Selig it was learned that operator #1 dropped his cell phone several times as he was holding it on scene. Both Officer Selig and operators #2 and #3 observed this. Operators #2 and #3 both expressed concern over the obvious odor of alcohol eminating from operator #1, and the fact that he was unsteady on his feet. I responded to NWH to follow up with operator #1 however he did not wish to speak further. He did express concern over the well being of all parties involved. Operator #1 was issued in hand the following citations: T1268426 - 90/24 OUI Alcohol, 90/24 Leaving the Scene (property damage, vehicle #2), and 90/24 Leaving the Scene (property damage, vehicle #3). T1268425 - 89/4a Marked Lanes violation. T1269216 - 90/24 Negligent Operation of a M/V (mailed after consultation with superiors) All streets mentioned are public ways in the City of Newton. 1354 Washington St will be the address used on this report and subsequent citations due to multiple locations being involved. Citations and paperwork have been placed in the prosecutor's mailbox. An immediate threat form has been filled out and faxed to Mass DOT. *Attention Police Prosecutor for blood work subpoena, if applicable*

The operator of MV#1 stated she proceeded to move forward when a school bus directly behind that vehicle also made a left turn making contact with MV#1. The operator of MV#1 honked her horn to get the school bus's attention with negative results. MV#1 sustained minor damages to its driver's side front bumper and fog light. The damage to the school bus should be to its driver's side rear bumper. There were no reported injuries to the operator of MV#1. There were no reports of any contracted school bus operators reporting an accident. The operator of MV#1 stated there were witnesses at the accident scene but she did not get any of their information. The operator of MV#1 was advised to contact her insurance provider.

He stated he thought he hit snow, but with the damage on the front right he realized he hit another vehilce and wanted to report it. Grady's statements (date, time, and location) matched with this hit and run. Grady was advised of the process.

On Friday March 15th 2019 at approximately 0730 hours I responded to the intersection of Beacon Street and Herrick Road for a report of a hit and run involving a bicyclist. The weather at the time of the accident was cloudy and the road conditions were dry. Both streets are public ways in the City of Newton. Upon arrival I observed the victim identified as Glenn Butcher (MA OLN S36552335), inside the ambulance getting medical attention. Butcher was getting a bandage applied to his left leg and complained of leg, back and neck pain. Butcher appeared visibly shaken and in pain. Butcher stated he had the green traffic light and was travelling eastbound on Beacon Street crossing Centre Street and then continuing straight crossing Herrick Road. Butcher stated MV1 was travelling westbound on Beacon Street and took a left turn onto Herrick Road and collided into his bicycle. Butcher states upon impact he flipped over his bicycle and landed on the hood of MV1, before rolling off and landing on the ground. Butcher states he was wearing his helmet which was present. Butcher states his bicycle ended up partially under MV1. Butcher states the operator of MV1 pulled his vehicle to the side of the road and got out of his vehicle and spoke to several witnesses briefly. Butcher states the operator then left the scene without properly exchanging information and did not wait for the police to arrive. Butcher described the vehicle as a dark gray Honda, and described the operator as an older white male, 5'10 approximately 80 years old with white hair. Butcher was also able to provide an email address for another bicyclist who witnessed the accident. The witness left his information with Butcher prior to my arrival. Fire fighters on scene were able to get the registration number of the vehicle from the witness (MA REG 4CX148) and gave it to Officers on scene. Butcher was transported by Medic 1 to NWH with minor injuries. His bicycle had visible damage to the frame area and skid marks were present on both tires. 19 photographs of the scene and the bicycle were taken and sent to the IT dept. email. Officer Gabriel transported the bicycle to the station where it was logged into Property & Evidence for safekeeping. Dispatch was able to query the registration which came back to a gray, 2010 Honda Accord registered to a Matthew Sugarman (MA OLN S38601397), out of W. Roxbury. Dispatch contacted the Boston Police Dept. and had them send an Officer to the address to try to make contact with Sugarman. Sugarman was advised to contact the Newton Police Dept. and I was able to speak with him on the telephone. Sugarman states he was driving the vehicle today and was involved in an accident with a bicycle. Sugarman states he was travelling westbound on Beacon Street and was taking a left onto Centre Street not Herrick Road. While speaking with Sugarman he seemed confused as to his location during the accident and was adamant that he was turning left onto Centre Street and not Herrick Road. I advised Sugarman that I was on scene with the first responding Officers and the accident clearly occurred at the intersection of Beacon Street and Herrick Road based upon the final resting place of the bicycle. Sugarman went on to state that his vehicle was already turning when the bicycle struck his vehicle and he believed it was the bicyclists fault. I advised Sugarman it appears the bicyclist had the right of way since it was traveling straight and that because his vehicle was taking a left turn, he failed to yield for on coming traffic at an intersection. When asked why he did not stay at the scene of the accident Sugarman stated he got out of his vehicle and spoke to the bicyclist and he "seemed okay". Sugarman states he then left the scene while the victim was still sitting on the ground. I asked Sugarman if he had ever been involved in accident before and he replied "no." I advised Sugarman that I am currently looking at his drivers history and see nine accidents that he was involved in. Sugarman stated "he has never been in accident like this before." I then asked Sugarman what the law states when you are involved in an accident and he was not able to provide me with an answer. I advised him that the law states you must stop and exchange information before leaving the scene of an accident. Again he stated the bicyclist seemed okay and "he had somewhere to be so he left." Sugarman has an extensive drivers history including nine surchargeable accidents, reckless operation, two different charges of leaving the scene property damage and two immediate threat affidavits filed against him. I advised Sugarman that I will be mailing him a citation for Ch89 s8 Fail to yield to oncoming traffic, and Ch90 s24 Leaving the scene of personal injury. I also filed an immediate threat form with the registry. I emailed the witness using the information he provided to the Newton Fire Dept that was on scene and received a phone call back from the witness identified as Bruce Adams. See witness report for his statements. I contacted the Bank of America located at 808 Beacon Street in Newton, as they are the closest business to that intersection with video surveillance. I spoke with George Walsh (781-834-8275) who stated they do not have any working cameras on that side of the building to the bank. For this report the bicyclist is entered as a non-motorist for validation purposes. Witness Bruce Adams (MA OLN S96398108) stated that he was riding his bicycle travelling behind the victim about 100 feet away. Adams states he was watching the victim and had a clear view of him as he travelled behind him, through the green traffic light heading eastbound on Beacon Street. Adams stated he witnessed MV1 travelling westbound on Beacon Street take a sharp left turn onto Herrick Road which turns into Union Street. Adams stated that the vehicle turned left colliding into the bicycle and there was no possible way the bicyclist had time to stop to avoid the collision. Adams stated the victim flew over his handle bars and fell on top of MV1's hood and then landed on the ground. Adams stated the victim's bicycle was stuck under MV1's front bumper. Adams stated the operator of MV1 backed his vehicle up with the bicycle still attached to his vehicle bumper and pulled MV1 over to the side of the road. The operator then got out of his vehicle and walked up to the victim who was laying on the ground visibly in pain. Adams stated the operator asked the victim if he was okay to which the victim did not immediatley reply as he was visibly shaken up. Adams stated the operator then walked back to his vehicle and attempted to leave the scene and that is when Adams stood infront of MV1 to stop him from leaving the scene. Adams stated he then removed the bicycle from underneath the bumper of MV1 and placed it on the side of the road. Adams stated the operator got out of his vehicle again and walked up to the victim and asked him "what do you want to do?". Adams stated the victim did not respond and that is when the operator got back into his vehicle and drove away, leaving the scene before any emergency response arrived. Adams stated he was able to memorize MV1's registration and stayed with the victim until police, fire and medics arrived. He stated he gave the fire dept the registration and his contact information prior to leaving the scene.

On 03/20/19 at 09:43 hours, I responded to the Avalon Apartments at Newton Highlands located at 99 Needham Street for a past hit and run that occurred at the east end garage exit on 03/19/19 at 14:10 hours. I spoke to Senior Community Manager Karen Schueler who showed me video surveillance footage from yesterday's accident. In the video, you can see a blue Subaru Forester attempting to exit the east end of the garage to the property. The operator of the Subaru is identified by Ms. Schueler as a resident of the building as Gertrude Weinstein. After Weinstein scans her garage pass to exit, she then over steers towards the right and strikes the overhead garage door opening device. She then continues to exit the garage scraping the entire passenger side of her car along the device that is attached to the wall. I viewed the garage door and there was significant damage as it became detached from the wall. The door had to be placed out of service. After I spoke to Ms. Weinstein who admitted to striking the side of the garage door but only thought there was damage to her vehicle. No citation issued as the underground garage to the building is private property and not a public way.

Foster stated at the date and time of the accident he was stopped in traffic a few cars back from the red light. Foster stated he felt something "restraining" his vehicle, and then backed up a few feet. Foster stated he then left the area. Foster stated he saw his passenger side door mirror was collapsed and saw a black truck next to him. Foster stated the truck didn't attempt to stop him and turned off the street away from him. Foster stated he didn't think it was a problem and left the area. Foster added that he thought the black truck was moving left towards his lane when he felt something "restraining" his vehicle. Foster stated he didn't believe his vehicle was involved in an accident. Foster stated there is no damage to his vehicle. Foster was advised to contact this insurance company. Foster reported no injuries. Foster was updated as the operator of MV1.

MV#1 stated while traveling Southbound on Centre St in the left turn lane at the intersection of Centre and Commonwealth ave. He stated he was at a complete stop waiting to turn left on to Commonwealth ave. He stated MV#2 was in the right lane adjasent to his vehicle when it took a sudden left turn on to Commonwealth Ave striking MV#1 in the right rear side oh his vehicle with the front left end of MV#2. He stated MV#2 then continued to drive behind him Eastbound on Commonwealth ave. MV#1 stopped on Read CT to evaluate the damage at which point MV#2 kept driving. MV#1 stated the car traveling behind MV#2 witnessed the incident and gatherd the plate information. The witness was not on scene upon my arrival nor did MV#1 gather their information. Newton Dispatch ran MA Reg 7EW346. The plate came back to a Black Honda Civic 2005 matching MV#1's description of the vehicle. The owner of the Civic was listed out of Brighton. I followed up at 78R Foster St Brighton, Upon arrival I noticed a black Civic matching the plate and description. The Civic had damage to the front end and damage to the back right side. I spoke with owner of the vehicle that stated he had not been driving today and had been at home. He did not know who was operating the vehicle at the time. Speaking with Medina the owner of MV#2 there was a significant language barrier issue. I will not be charging owner of MV#2 due to the vehicle damage not being consistent with MV#1 statements, as well as proof of operation. MV#1 was not towed. Operator of MV#1 stated he did not sustain any injuries at this time. Owner of MV#2 did not have any injuries to him consistent with being in a MV accident.

MV1 was parallel parked in front of Bank of America on Walnut St when he began to pull out, MV2, ( suspected to be MA Reg:8WX995) hit MV1 on his driver's side door. MV2 backed up in the road and then sped off without stopping to give his information. MV1 operator, Marcus Klevan, came into the police station to report this hit and run which occurred around 1430hrs today. MV1 operator and passenger were able to identify the license plate of the vehicle as it left the scene. The plate came back to a 1997 Toyota Tercel, blue, registered to John Twinomujuni out of Waltham. Waltham PD was notified and was unable to locate the vehicle at this time. UPDATE: Waltham PD was requested to check again for this vehicle at it's registered address tonight, 5/1/2019. This check resulted in negative results for the vehicle. No citation will be issued at this time until there is verification that this vehicle was indeed involved in this accident.

Owner of MV1 states his vehicle was backed into a parking spot at the JCC, 333 Nahanton St. Sometime between 0720 to 0815 his vehicle was struck on the drivers side bumper (minor damage). There was possibly some white paint transfer however it was cleaned off prior to my arrival. I contacted the JCC, they stated they may have cameras, however the only person that can access them is not working (617 558 6552 - Brian Morris). There were no witnesses to the accident and it was not reported until later in the day.

I observed the post to be leaning due to the impact. The witness was unable to get a plate number, but was able to tell me that it was a commercial truck with a California registration. Dispatcher Brunelle notified Dagle to fix the light post. Photos were taken of the pole, and submitted to the IT bureau.

Operator of MV1 (Ma Reg 35T230) stated she was exiting the JCC parking lot, turning right onto Nahanton Street when a black SUV (No further description was provided), came around the corner, heading Southbound on Nahanton Street when MV2 (Black SUV) struck MV1's driver side front bumper. Operator of MV1 stated MV2 was traveling at a speed greater than it should around the corner prior to striking her vehicle. Operator of MV1 stated she got out of her vehicle to check on the damage and saw MV2 stopped further down the road. When she got back into her vehicle to meet with the operator of MV2, the vehicle was no longer there. No direction of travel of provided. Operator of MV1 and her daughter were not injured. There was minor damage to the front driver side bumper of MV1. I spoke with the JCC Facility Manager, Brian Morris (617-775-1719) who stated he will check the cameras to see if the cameras recorded the accident. I provided Mr. Morris with the time of the accident and my name and advised him to contact NPD if he has any information to provide. Prior to clearing, I canvassed the area for the vehicle with the description provided, yielding negative results.

On 05/10/19 I met with Luiz A. Lopes who stated his vehicle ( Mass Commercial Reg# T83695 White Toyota Tacoma P/ U) was struck while parked in the lot around 07:00. Lopes said he didn't call the Police right away because he wasn't sure his car was struck there. The truck sustained a cracked front bumper and the fog light was popped out. We went inside of National Lumber and spoke with a manager who said he would contact G.M. Greg Assarian and have him look at the security footage. On 05/14/19 at approximately 14:00 the Police Station received a call from Lopes who just got off the phone with Assarian who found some security footage of the incident. I then traveled over to National Lumber and met with Assarian. He showed me the video and it appears that a newer grey four door Chevrolet truck backed into Lopes truck around 06:58 of 05/07/19. The Massachusetts plate is unreadable but an older White heavy set male gets out of the truck and walks into the store. After parking the truck the male can be seen checking his own truck bumper for damage. The male is wearing a baseball hat and sun glasses and cannot be identified on the video. The male does however walk with a noticeable limp. Assarian will show his picture to his employees and contact me if the employees are able to identify the male driver. I called Lopes back and apprised him of my findings. ******************UPDATE**************************************** As of today 05/20/19 I have yet to hear back from anyone at National Lumber so I will end this investigation and label it a hit and run crash.

Upon arrival I spoke with Ethan Smelson who stated he parked MV#1 in the parking lot and then left in another friends vehicle. When he returned to the McDonalds he noticed there was minor damage to the front driver side bumper to MV#1. There was no one inside of MV#1 at the time of the accident and there was no other information on the vehicle that might have hit MV#1. The vehicle takes a left turn onto Goff Street, parks, and a male party exits the vehicle and delievers a package to 16 Crehore Drive. There is no date or time associated with the video and I am unable to make out a license place number or identify the driver. I am unable to determine if this vehicle made contact with MV1.

On July 3, 2019 at approximately 00:08 hours I responded to 25 Crosby Rd for a report of a hit and run that had just occured. Upon arrival, I spoke with Nicholas Giamalis who reported that he heard a bang and went outside and observed damage to MV1's left rear bumper. MV1 was parked facing southbound in front of 25 Crosby Rd. Nicholas said that a gray Chevy sedan driving northbound on Crosby Rd towards Commonwealth Ave. Officer Kayla Donahue canvased the area with negative results. Nicholas said that he was the last person to use the vehicle but the vehicle is owned by his father, Stephen Giamalis. Based on the travel direction of the unknown vehicle and the location of the dent, it is likely that MV1 was backed into by an unknown motor vehicle. It does not appear that anyone witnessed the actual crash and there did not appear to be any cameras in the area.

On 7/15/19, I responded to the West Newton commuter lot to speak with a party who had her vehicle struck in front of 305 Centre St. The operator of MV1 (MA Reg:4YK555) stated that she was travelling southbound on Centre St when a white Mercedes SUV came into her lane and collided with her drivers side. On Monday, July 15, 2019, while assigned to Traffic unit N525, I responded to the area of Beacon Street and Paulson Road, Newton, for a report of a hit and run motor vehicle accident involving a bicyclist. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street and Paulson Road are both public ways maintained by the City of Newton. I spoke with the injured bicyclist in front of 165 Paulson Road, Alexander Mendonez. Mendonez stated he was traveling Westbound on Beacon Street on his bicycle near Paulson Road. Mendonez stated he observed a late model gray sedan "inch to a stop" on Beacon Street (W) at Paulson Road. Mendonez stated he did not remember if the vehicle had it's right directional on at the time. Mendonez stated as he was traveling through the intersection with the vehicle on his left, the vehicle took a right turn onto Paulson in front of him. Mendonez stated he attempted to brake and veer to the right of the vehicle, but was unable to avoid the collision. Mendonez stated his left side made contact with the rear passenger door/rear fender area of the vehicle. Mendonez fell to the roadway and the vehicle left the area Northbound on Paulson Road. Mendonez was unable to see who was operating the vehicle. Newton Medics responded and treated Mendonez for injuries to his left knee/arm area. Mendonez signed a patient refusal with Newton Medics. I observed minor damage to the left handlebar area of Mendonez's bicycle (black Trek). Mendonez was wearing a white helmet at the time of the accident with a blue shirt and white and black shorts. Pictures were taken of the area and bicycle and submitted to the IT Bureau. Mendonez was able to provide a partial Massachusetts registration of "7BS32". I attempted run this plate with multiple combinations. I searched the area for a vehicle matching the description given by Mendonez with a negative result. On July 16, 2019, I spoke with a witness to the incident Mark O'Hearne. O'Hearne stated he was traveling Westbound on Beacon Street a few car lengths behind where the accident occurred. O'Hearne stated he observed a bicyclist on the right side of the road abruptly stop and fall to the roadway. O'Hearne stated he observed a vehicle in the path of the bicycle. O'Hearne stated the vehicle did not stop after the bicyclist fell and took a right onto Paulson Road from Beacon Street. O'Hearne stated he followed the vehicle involved in the accident to Commonwealth Avenue, and while doing so took a picture of the vehicle's license plate. O'Hearne stated he did not observe who was driving the vehicle. The Massachusetts registration provided to me by O'Hearne is 7BS322. This vehicle comes back to a 2010 Volkswagon Jetta registered to Peter Giorgi (S20952090) out of Paxton, Ma. I spoke with Peter via telephone. Peter acknowledged he is the registered owner of the vehicle, but stated his daughter was using the vehicle and was in the Newton area. I spoke with Lindsey Giorgi (S35711247) via telephone. Giorgi stated she was operating MA: 7BS322 in Newton yesterday, but does not recall being in an accident. Giorgi stated she is not familiar with the area and does not know the area where the accident occurred. Giorgi stated there is no damage to her vehicle and stated she does not believe she hit anyone. Giorgi emailed me 4 photos of her vehicle where I described the damage would be. I observed a small area of minor damage to the rear passenger side door area of this vehicle. After speaking with all parties involved, I am requesting a hearing in Newton District Court for Chapter 90, Section 24 (Leaving the Scene, Personal Injury). Giorgi was advised and mailed Massachusetts Uniform Citation T1269847. All photos obtained from O'Hearne and Giorgi were submitted to the IT Bureau.

On 07/18/2019 Gladys Bisono came into NPD Headquarters to report a past-hit and run. Bisono states that between 1500hrs on 7/13/2019 and 1100hrs on 7/14/2019 her leased vehicle (Vehicle 1) was parked and unoccupied on the eastbound side of Border St near 85 Border St. During this time frame an unknown vehicle (Vehicle 2) struck vehicle 1. As a result of the impact Vehicle 1 sustained damage to its front end and front bumper. Vehicle 2 left the scene with out leaving any identifying information. No injuries were reported. No vehicles were towed. Bisono has since reported the crash to her insurance company who advised her to file a police report. At this time the identity of Vehicle 2 and its operator is unknown. Motor Vehicle 1 (MV1) was traveling northbound on Centre St. at the intersection of Walnut St. when motor vehicle 2 (MV2), which was also traveling northbound on Centre St., attempted to go straight from the left turn only lane. MV2 then sideswiped MV1, running MV1 off the road and into the curb. The operator of MV1, Drew Brookins, stated that the operator of MV2 pulled over at first, but when traffic cleared, immediately got back into the car and left at a high rate of speed heading northbound on Centre St. Mr. Brookins attempted to follow MV2, and observed MV2 take a right onto Paul Street (eastbound), but was unable to continue to follow MV2, as MV1's front drivers side wheel was blown out as a result of colliding with the curb. MV1 sustained drivers side damage, as well as a blown out right drivers side tire. MV2 likely sustained moderated drivers side damage, and possibly damage to the rear bumper. Mr. Brookins was able to get the license plate of MV2 (MA Reg. 9LE764), which comes back to a female (Desiree Allman) out of Braintree. Mr. Brookins described the operator of MV2 as a white male with a thin build, dirty blond hair and scruffy beard. Dispatch contacted Braintree PD who was able to provide a telephone number that is believed to be Ms. Allman's. I attempted to contact Ms. Allman, however, the number went directly to voicemail. I left a message asking Ms. Allman to call the NPD so that we could find out who was operating her vehicle at the time of the crash. Traffic Bureau Update (Officer Gaudet): On August 6, 2019, I attempted to make contact with the registered owner of MV2, Desiree Allman (S85529796), with a negative result. A Hit and Run inquiry was mailed to the Allman. As of August 8, 2019, I have left several voice mails for Ms. Allman, she has not returned my calls. Ms. Allman had a hearing today in Hingham District Court. I contacted them to confirm we have her current contact information.

Operator of vehicle 1 stated she was stopped at the stop sign on Crescent St at Auburn St when she saw 2 vehicles coming eastbound stopped to let her out. As she pulled out taking a left onto Auburn St she was hit on the drivers side by another vehicle which came from the opposite direction. The operator told everyone he was pulling over then quickly took off westbound on Auburn St towards Commonwealth Ave. The passenger of vehicle 1 stated that the driver of the vehicle was a black male in his early thirties driving a green possibly 2010-2012 Jeep Cherokee with a possible CT reg# of AP1042 or AP81042. He also stated that the driver looked like someone he tried to purchase a car off of in Waltham four months ago but had no further information. Three witnesses to the accident were in the two vehicles stopped to allow vehicle 1 to pull out of Crescent St. All three parties gave the same story that vehicle 1 was almost completely out of Crescent St when the green jeep coming westbound quickly accelerated and looked as if he was going to go around her but hit he drivers side then took off westbound. The vehicle description and possible plate were given out to surrounding towns and state police. Operator of vehicle 1 was transported by Cataldo to NWH for minor injuries. Vehicle 1 was towed from the scene by Todys.

On 08/06/19 at 12:34 hours, I responded to Industrial Place by The Lakeshore Learning Store at the request of Newton Police Officer Justin March who witnessed an accident in the parking lot of 300 Needham Street. Officer March was working a road construction detail at the time in the City of Newton. He states that at approximately 12:20 hours he observed MV1 exiting the parking lot of 300 Needham Street( a drive way to a building that the public has access to). There is an island in the middle that separates the entrance and exit of the lot as it goes out to Needham Street. As MV1 was exiting, he drove up on the island and pushed two large stone builders out of place. There was also damage to numerous different flowers planted in the ground. MV1 then exited the lot and went eastbound on Needham Street. Officer March then followed in his vehicle and kept beeping his horn to alert the driver to stop. The operator turned on to Industrial Place and pulled over. Ofc. March attempted to speak to the operator, Gelvin Perez-Ramirez but there was a language barrier. Next, Ofc. Calderon responded and assisted with Spanish translation. He asked the operator what occurred and he stated that he never saw the island. Due to the fact that the operator left the scene of the accident and never reported it, he was issued MA Uniform Citation # T1445351 for Ch. 90/24/C. MV1 sustained damage to the two exhaust pipes on the passenger side of the truck. There is also possible undercarriage damage. Ofc. M. Wade took photos of the scene.

She took a photo of MV2 which was a black Toyota Highlander bearing MA reg.# R85045. She described the operator as a white female in her forties with possibly dark curly hair. I observed minor paint damage and scratches to the driver's side rear quarter panel area. The operator is unsure if that damage is from the accident. The owner of MV2, Steven Masterson has a residential address of 40 Wild Acres Rd in North Attleboro, MA and a mailing address of 34 Central Street Foxborough, MA. A telephone search revealed a phone number of (269)857-7814 for him out of Aiken, SC and the Foxborough address but the number is not in service. UPDATE: I was provided a phone number for Masterson from N. Attleboro PD. On 08/16/19 at 12:45 hours, I spoke to Sheila Masterson who stated she was driving MV2 at the time of the reported accident. She stated that she was unaware that she struck MV1 and saw the other operator looking at her but thought nothing of it. Due to the accident being very minor or possibly no damage at all, no further action will be taken. Masterson was provided with the other operator's information.

Owner of Motor Vehicle #1 stated that he was exiting the gym at Planet Fitness and he noticed that someone was leaving a piece of paper on his front windshield. He was approached by the man who told him that he witnessed a Gray Dodge Ram bearing MA Reg 9JGD90 strike his Jeep Grand Cherokee that was parked and unoccupied causing moderate rear drivers side damage. MA Reg 9JGD90 came back to a 2018 Dodge Ram 1500 Gray in Color and registered to a Kenneth Doucette out of 41 Thaxter Street, Newton MA. I was able to locate several telephone numbers from our in house system however those numbers were no longer in service. I made several attempts to make contact with Mr. Doucette at his residents and had negative results. N491 Officer D. Healy also made an attempt to make contact with Kenneth Doucette later on in the evening and had negative results. I left a voicemail message to the witness and have yet to receive a return call for questioning. This will be further investigated on my next tour of Duty. **UPDATE Wednesday 09/04/19** At approximately 10:00 a.m. I went to 41 Thaxter Street to once again attempt to make contact with Kenneth Doucette and had negative results. I left another voicemail for the witness Claudio Silva and had negative results. I then spoke to Traffic Officer M. Gaudet to inform him about this incident and he stated that he will follow up with this investigation. Traffic Bureau update (Officer Gaudet): On September 4, 2019, I attempted to make contact with the witness to the crash, Claudio Silva, with a negative result. I made contact with the registered owner of MV2, Kenneth Doucette (S80920300). Doucette stated he was operating MA: 9JGD90 at the time of the accident. Doucette stated he pulled into a parking spot next to MV1 and stated he didn't think he "hit it hard". Doucette stated a short time later he left the area and when he got back to work he observed damage to his own vehicle. Doucette stated he had damage to the front passenger bumper area. Doucette stated he returned to the crash scene to see if the other vehicle had damage but it was gone. Doucette was advised and provided Massachusetts Uniform Citation T2016422 in hand for Chapter 90, Section 24 (Leaving the Scene of Property Damage, MV).

On 9/08/19 Kevin Coston came into NPD Headquarters to report a MV/Ped with Injury that had occurred on 8/23/19 at about 06:15 in the parking lot of Whole Foods (916 Walnut Street). The RP stated that he had left work (Whole Foods) at the back receiving door and was walking to his car, he bent over to take his knee pads off, while he was doing this he was struck by a delivery van that backed into him. The driver took out his headphones and said sorry before driving away. RP stated that he told the driver that he had been hit but the driver just drove away. RP went to hospital (Faulkner) on his own for lower back pain and has not been to work since. RP was able to take picture of the side of the delivery van. The van belonged to Shortstop Distribution Inc (508-655-5948). RP left a message that morning. He was texted by Gedeon (786-352-6695) who stated there was a language barrier and he didn't understand what had happened. RP advised Gedeon to talk to his lawyer or the police. Exact location of accident unknown. Diagram is a estimate. Report printed out and placed in traffic box for follow up. Traffic Bureau Update (Officer Gaudet): On Monday, September 9, 2019 I attempted to make contact with the operator if MV1, Gedeon Duarte, with a negative result. A Hit and Run motor vehicle inquiry was mailed to Duarte. I spoke with the injured party, Kevin Coston, via telephone on September 9, 2019. Coston stated he is an employee at the Whole Foods located at 916 Walnut Street, Newton. Coston stated on August 23, 2019 a black van backed into him as he was kneeling on the ground taking off his knee pads in the loading area of the store. Coston stated he approached the driver who remained in the vehicle to speak with him after he was hit. Coston described the driver as a heavy set Spanish male. Coston stated the operator of the vehicle rolled down his window, took his headphones out of his ear, said sorry, and left the area. Coston stated he contacted the Newton Police to report the accident but was told he needed to come in person to file the report. Coston stated he was unable to come to the station to file the report until September 8, 2019. On Friday, September 13, 2019, I spoke with Gedeon Duarte at the Newton Police Department. Duarte stated he spoke Portuguese and did not speak English. With the assistance of David Sargent from the Language Line (1-800-225-5254) translating, I was to speak with Mr. Duarte in the Traffic Bureau. Duarte stated he was making a delivery at Whole Foods and was waiting for another truck to leave so he could leave the area. Duarte stated a male party (Coston) came up to his driver door and was yelling him. Duarte stated that due to language barrier he was unable to understand what the person was saying to him, but stated the male party was yelling, "Fuck you! Fuck you!" Duarte stated he did not know what was going on and stated he left the area to allow the male party to calm down. Duarte stated he returned a minute later the male party did not want to speak with him. Duarte stated he returned a second time later in the morning and Coston was not there. Duarte stated he received a call from his employer stating Kevin Coston had called for him and he was provided with Coston's phone number. Duarte stated he texted Coston to find out what happened that morning. Duarte showed me a text message exchange from August 23, 2019 at 9:03 A.M. between himself and a party listed as "Kevin Whole". In the text messages, Duarte stated he is the driver of the van and that his supervisor called him. The other party responded and stated "Talk to the police and my lawyer". Duarte stated that he did not understand what happened and that he returned to whole foods. Duarte ended the text exchange with "927535425 my number of policy..." Duarte stated he had no idea he made contact with Coston and stated at the time of the alleged accident he did not hear/see/feel anything otherwise he would have stopped. Duarte's information has been updated in this report to reflect him as the operator/vehicle involved. Duarte also submitted pictures of the rear of his vehicle, a 2008 black Ford E350 (MA: 8NY171), from the day of the accident. Photos were submitted to the IT Bureau. Officer Wade went to the Whole Foods located at 916 Walnut Street, Newton in an attempt to view surveillance footage of the accident. Officer Wade stated the surveillance camera did not capture the incident and the van not viewed at all.

Upon discovering the damage, the owner of Vehicle 1 canvassed the area and located a surveillance camera at the Village Bank 332 Walnut St. The owner of Vehicle 1 was informed by Village Bank that they do not release surveillance video's to civilians and that he would need to report the incident to the police. It should be noted that this crash occurred on 09/11/2019 and was reported at the front desk of NPD Headquarters on 09/12/2019. I will be contacting Village Bank on 09/13/2019 regarding the surveillance video. Update: On 09/13/2019 I spoke with Lisa Bocabella of the Village Bank. Ms. Bocabella states that the exterior surveillance video at their Newtonville Branch only captures images of their front doorway and the adjacent sidewalk. Ms Bocabella states that their surveillance did not capture this crash. At this time the owner and operator of Vehicle 2 is unknown. The owner of Vehicle 1 was notified of these results.

This crash occurred on 09/12/2019 between 1730 and 1800 hrs. No injuries were reported and no vehicles required a tow. The owner of Vehicle 1 Andrew Horvitz came to NPD Headquarters on 9/13/2019 to report the crash. Horvitz stated that Wegmans Supermarket may have video footage of the crash but would not release it to a civilian. I called Wegmans and spoke to Store Manager Chrissy Delmastro 617-762-2000. Delmastro stated she will notify her security team who will contact me back some time next week. I also contacted Dahn Pham 617-795-5999 of Chestnut Hill Square Security to inquire about their security footage. A message for Pham to contact me next week. Rizza stated he waited for the crosswalk signal to turn on. When it finally turned on to walk (white walking person) he got on his bike to start peddling. At this time, he observed a black vehicle traveling southbound traveling towards him. Moments later, Rizza was struck by the vehicle. Rizza rolled over the vehicle and hit the pavement. Rizza stated the vehicle took off, but he was unsure where it went (Mass Pike, Newton Corner, or Washington Street). The medics looked at Rizza and he was alert and had a minor laceration to his head. Rizza's mother Domenica Rizza picked him up and brought him to NWH. I drove around the area and could not find a black Honda sedan with damage to the front of it. As of now, I do not have anymore information. This report is a hit and run. Traffic Update (Officer Gaudet): Myself and Officer Anderson have both canvassed the area of the crash for identifying physical evidence from the vehicle that fled the scene with a negative result. We have also reached out to local business's in an attempt to secure surveillance footage of the crash with a negative result. The reporting party, Doris Rui, told me she was operating her husbands vehicle (Mass Reg # 495DT4 Toyt Rav4 Blue). Doris was driving north on Centre St at approximately 1500 hours. She stated the traffic light turned green and she started to travel through the intersection of Centre Street and Walnut Street. Both streets are public ways in the City of Newton. At this time she felt a vehicle crash into the rear of her Toyota. Doris pulled over to the side of the road on Centre St to exchange information and examine the damage. Doris stated The owner/operator of the other vehicle (later identified as Jonathan Wolfe) left the scene of the accident without leave any of his information. I placed a phone call to Wolfe's cell but he did not pick up his phone. Video Clips captured from Doris phone of the accident were uploaded to this report. Vehicle 1 (Mass Reg # 1FSK61 Honda Civic) suffered major damage to front ride side of the vehicle. Vehicle 2 (Mass Reg # 495DT4 Toyota Rav4) suffered major damage to the rear bumper of the vehicle. The operator of Vehicle 1 (Jonathan Wolfe) was charged with S90/S24 Leaving The Scene Of Property Damage (See Incident Report # 19039626). Both vehicles have insurance, and there were no injuries.

On September 30, 2019 at approximately 16:56 I responded to a reported hit and run crash at 195 Sumner St. Dispatch gave out a description of the vehicle that fled, described as a red pickup truck with a possible partial plate of" MVNA". I, along with several other units checked the area for the vehicle with negative results. I responded to 195 Sumner St. and spoke with the operator of MV 1, who explained that he was traveling Northbound on Langley Rd onto Sumner St when a red pickup truck attempted to pass the RP from behind. Due to Sumner St being a narrow, one way street, the pickup truck cut back in front of MV1 and struck the front driver's side of MV1 with the rear passenger side of the pickup truck.The pickup truck proceeded down Sumner St. towards Centre St. without attempted to stop. The operator of MV1 did not know the make or model of the vehicle, but described it as a full size pickup truck that was red/maroon with a black tailgate with a large "fox racing" sticker on it. The description of the vehicle that fled was broadcasted to units canvassing the area, but was not found.

On 10/16/2019 I took a motor vehicle Hit and Run report at the front desk of NPD Headquarters. The owner of Vehicle 1 states that on 10/15/2019 at approximately 1600hrs Vehicle 1 was parked an unoccupied facing southbound in a parking space of the Elm St Municipal Parking Lot 100 Elm St. Vehicle 1 was then struck by Vehicle 2 who was pulling out of an adjacent parking spot. As a result of the impact Vehicle 1 sustained minor damage to the passenger side rear bumper. Vehicle 2 left the scene without leaving any identifiable information. No injuries were reported, no vehicles were towed. The owner of Vehicle 1 stated that he found a note on his windshield from an anonymous witness stating Vehicle 1 was struck by a gray Honda CRV MA Reg 665SW5. No other identifying information was included on the note. An RMV query of 665SW5 revealed a registered owner of Marylou Gair of Boston MA. I was unable to obtain any further contact information for Gair. At this time the operator of Vehicle 2 is unknown. This crash report will be forwarded to the Traffic Bureau for further investigation.

MV 1 was traveling northbound on Walnut street and struck MV 2 traveling in the opposite direction. MV 2 was traveling southbound on Walnut Street and was struck by MV 1 traveling in the opposite direction. The accident was reported at Walnut St and Beacon St. MV 1 reportedly crossed the double yellow lines into MV 2's lane. There was a BOLO announced over the air for MV 1 because it was reported as leaving the scene. MV 1 was found and stopped at Walnut St at Homer St. It was determined that the operator of MV 1 was under the influence. The operator was placed under arrest. There were no injuries to either party. The children were in MV1. The children were transported to Newton Wellesley Hospital and assessed for possible injuries. Both vehicles were towed by Tody's. MV1 sustained significant damage and was not able to be driven away. The operator would have been aware of the accident based on the level of damage.

On 11/27/2019, at 1611 hours, I responded to the area of 69 Waltham St, Highlander Farms, for a two car hit and run accident. Upon arrival, I observed MV1 to have damage along the drivers side. The operator of MV#1, identified as Frank Vanmierlo states that he parked his MV in the Marriott parking area at approx. 7:45AM on 12/9/19. When he returned to his unoccupied MV he found MV#2 making contact with the rear of his MV and his MV was now moved slightly from its original parking spot. MV#2 was unoccupied when Mr. Vanmierlo returned to his MV and we could not locate the registered owner on the Marriott property. I then spoke with the operator of MV#2 by telephone, identified as Jonathan Lang. Mr. Lang stated that he parked MV#2 in the Marriott parking area at approx. 7:45AM and was not involved in a crash with any other MV while in the parking area. Mr. Lang states that there was another MV parked and unoccupied in the parking space in front of him when he turned left into his parking spot. Marriott security states that there are no cameras video taping the exterior of the Marriott including the parking area. Interior videotape provided by the Marriott confirms that Mr. Lang entered the front lobby before Mr. Vanmierlo. Upon further investigation it was determined that an unknown person had informed Mr. Vanmierlo that a Red Acura had struck his MV while parked in the parking area. Mr. Vanmierlo also confirmed that there was new damage to the front of his MV. No other information available at this time. It would appear at this time that unknown MV#3 backed into the front of MV#1, pushing MV#1 into the front of MV#2.

MV1 stated he already filed an accident report with the RMV and notified his insurance company. I was able to locate an email address for the company and emailed a request for information but have not received a response. I advised MV1 of my investigation results and he was satisfied. MV1 stated the damage to his vehicle was minor.

Operator of MV1 states she had the green signal when MV2, described as a dark colored pick up truck, ran the red light traveling northbound from the off ramp of Rt. 95 southbound/Quinobequin Rd. and crashed into MV1 head on. Operator of MV1 states MV2 fled the scene traveling northbound in the Do Not Enter lane traveling towards Grove St. Operator of MV1 states MV2 sustained front end damage, while MV1 sustained damage to the right front fender and was towed from the scene. Operator of MV1 sustained a wrist injury and was transported to Newton Wellesley Hospital. While investigating the scene, the license plate (MA Reg: 1YF845) from MV2 was left behind from the crash. The registration plate came back on a gray 2017 Honda Ridgleline pick up truck, which matches the description of the vehicle involved in the crash. The vehicle is registered to Andrew Wiggin out of 32 Marlborough Rd Salem, MA. Two witnesses, Lisa Duncan and Jennifer McQuillan, witnessed the crash. Ms. McQuillan states she was behind MV1 when she observed MV2 run the red light, crash into MV1 head on and fled the scene northbound in the Do Not Enter lane. Ms. Duncan states she was a few car lengths behind MV1 traveling on the right hand side of MV1. Ms. Duncan also states she observed MV2 run the red light, crash into MV1 head on and fled the scene northbound in the Do Not Enter lane. Both Ms. McQuillan and Ms. Duncan believe a male was operating the vehicle.

THE OPERATOR THEN WENT INTO THE VEHICLE AND LEFT THE VEHICLE UNOCCUPIED WHILE SHE WENT SHOPPING. A SHORT WHILE LATER THE OPERATOR CAME BACK TO THE VEHICLE AND OBSERVED FRESH DAMAGE TO THE REAR PASSENGER BUMPER WHICH APPEARS TO HAVE BEEN DONE WHEN AN UNKNOWN MV PARKED TO THE RIGHT OF HER VEHICLE EXITED THE PARKING SPACE. THERE WERE NO WITNESSES AND NO SURVEILLANCE FOOTAGE OF THE ACCIDENT.

On January 9, 2020 at apprrox. 1430 hours I responded to the parking lot of 333 Nahanton St " JCC greater Boston" for a report of a past hit/run. Upon arrival I met with the RP Mr. Eugene Magier d.o.b. 7-7-61 35 Brandeis RD Newton MA 02459. He stated he parked his vehicle in the parking lot of 333 Nahanton St JCC building at approx. 0930 hours. He stated he returned to his vehicle at approx. 1030 there was a vehicle next to him parked too close so he went through the passenger door to get into his vehicle. He stated when he went to his next destination, he exited his vehicle and observed the damage. I observed the damage to the driver side rear near the wheel well to the pass door. He also stated he went back to the parking lot where he parked and found a blinker light next to where he parked and the color of the blinker light matched the partial color of the damage to his vehicle. The blinker had "Toyota" on it and the following partial numbers "KOITO - 48-5 SAE EAP2 96" I canvassed the area and observed camera in the possible area of the crash. I went to the JCC and met with Director of facilities Brian Morris 617-558-6552. We observed some video in that area that may be visible if the vehicle goes against the building to park. Mr. Morris is going to watch the video to see if there is a vehicle within the time frame and can observe the crash. He will contact NPD of his findings. I advised the owner of the process. I then left without further incident.

The owner of MV#1 stated she had parked her vehicle in the parking lot of 1314 Centre St and went into the store (Holly Cleaners) to pick up her dry cleaning. The owner of MV#1 stated her friend "Rosemary" who was sitting in the front passenger seat of MV#1, observed the vehicle parked next to it back out of its parking spot and struck MV#1. I spoke to the manager of Holly Cleaners and she was able to provide me with the customer's contact information that drove MV#2. The manager of Holly Cleaners was able to show me video footages of the accident. The video shows that operator of MV#2 attempted to back out of his parking spot making a sharp turn and leaving the area. The video then shows the passenger of MV#1 get out of the vehicle and looked at the damages. I am unable to determined if contact was made between the two vehicles but it was highly possible that contact was made. MV#2 appeared to be a gold colored "Audi" SUV operated by a Fred Ellis of 36 Sallbury Rd in Brookline MA (617 731-3358). I am unable to view a registration or locate a registered vehicle to a Fred Ellis at this time. The damage to MV#1 is to its driver's side rear door. It displayed a light color mark (Minor damages). There were no reported injuries to all parties involved. The front passenger of MV#1 was only known as "Rosemary" (Unknown last name, address or D.O.B). I contacted the operator of MV#2 and left a voicemail. To be further investigated pending statements from the operator of MV#2. On 01/17/2020 at 09:45 I spoke to Alfred Ellis and he stated that he was at the Holly Cleaners parking lot and could not reverse due to MV#1 parking so close to his vehicle. Mr. Ellis stated he unsure if he had made contact with MV#1. Mr. Ellis was forthcoming with his statements and provided me with his vehicle and license information. He stated he was not injured and I provided him with this accident report number along with information of the other involved vehicle.

There are no cameras from the Ballet School in the direction where Berleus parked. ************************UPDATE*************************** As of my 02/09/20 at approximately 11:00, I have yet to hear back from Manager Jim Boucher or anyone else from Stop & Shop. On Friday, February 7, 2020 while assigned to Traffic unit N525, I responded to the rear parking lot of 1 Boylston Street, Newton for a report of a hit and run motor vehicle crash. I spoke with the operator of MV1, Hong Zhang (S39408212). Zhang stated while operating her 2019 Volkswagon Tiguan (MA: 6YJF90), she was attempting to back out of a parking spot in the parking lot behind 25 Boylston Street, Newton. Zhang stated while backing out of the spot she crashed into an unknown vehicle traveling Westbound through the parking lot. Zhang stated the only information she had for the other vehicle was that it was a large red and black truck. I observed damage to the rear trunk window of MV1. Zhang reported no injuries. Zhang stated she was unsure if she hit the vehicle or if something fell off of it as it was passing by. I went to several local businesses in the area in an attempt to view surveillance footage of the area where the accident occurred with a negative result. While canvassing the area for the other vehicle involved, I observed a red dump truck (MA CON: T71619) removing snow on the property next to Hammond Pond Parkway. The operator stated he has been working down that end of the parking lot for most of the morning and stated he was not involved in a crash. I observed no fresh damage to the vehicle consistent with a crash.

On February 10, 2020 at approximately 1911 hours, while on patrol in Newton Centre Sq, I was approached by a woman who wished to report a past MV hit and run. Ms. Eileen Kavall stated last Thursday(02/06) between the hours of 1745 and 1830, her MV was parked in a handicap spot in front of the Star Market at 1 Boylston St. While parking her vehicle, she noticed a large white SUV parked in the spot directly next to her. She stated this vehicle was also in a handicap parking space but did not see a handicap sticker on the vehicle. When Eileen exited the store, she observed damage to her front left fender and the SUV was gone. I also observed this damage. Eileen did not have a number plate for the MV that hit her but stated the Star Market may have cameras facing where the incident occurred. I then provided Eileen with an accident report number and went to Star market to speak with the manager. At Star Market, I spoke with Janet Yaksic, store manager, who stated she remembered speaking with Eileen about the accident. Janet stated that she would have to contact the store's director in order to contact the loss prevention employees who do not work on site. The loss prevention employees would be able to review any possible footage by the end of the week and contact Newton Police with their findings. I left my name and contact info with Janet. This accident is still under investigation pending a review of the Star Market footage. Update: 02/22/20 I have not received any video surveillance from Star Market. RP will be advised to pursue with their insurance company.

The operator of vehicle 1 stated she was traveling West on the Washington Street bridge over the Mass Pike when vehicle 2, a gray SUV attempted to change lanes, as this happened the front passenger side quarter panel of vehicle 2 came in contact with the rear drivers side quarter panel of vehicle 1. This caused minor scratches to vehicle 1. The operator of vehicle 1 attempted to pull over and exchange information with the other driver. The operator of vehicle 2 continued driving away and got onto the Mass Pike, during this process the operator of vehicle 1 and the passenger of vehicle 1 were able to get the license plate number of vehicle 2 which was NY REG:T797800C. In an attempt to locate and identify the driver of vehicle 2 I ran NY REG T797800C which comes back registered to a short term rental company called Flexdrive. I called a phone number on their website 8663539399 which is listed as the number to call if you are in an accident with one of their vehicles. I was only able to speak to a service technician, a Tom Louis of ARI Fleet Service 18002272273, who told me the vehicle was out of the Flexdrive North Dighton Mass hub located at 123 Williams Street North Dighton MA 02764.Tom also mentioned the location would most likely not be open again until Monday. He did not have any other information regarding who may be operating the vehicle. I have attempted to find contact information on this North Dighton location which I have had negative results so far. I have also emailed the Flexdrive customer service department. To be followed up on. Traffic Bureau update (Officer Gaudet): On Tuesday, February 25, 2020, I made contact with the registered owner of MV2, Flexdrive. The representative from Flexdrive stated MV2 is registered to the company but Lyft uses their vehicles for ride sharing. I contacted Lyft and spoke with a party named Jonas from their support team. Jonas stated he would make contact with the operator of MV2 and ask them to contact me to give a statement. Lyft stated they would also conduct their own internal investigation. On Wednesday, February 26, 2020, I spoke with the operator of MV2, Gary Procaccini (S36066201). Procaccini stated he was operating a 2019 Santa Fe (NY Reg: T797800C) as a Lyft driver on the evening of February 22, 2020. Procaccini stated he was traveling on Washington Street (W) towards the Massachusetts Turnpike entrance in West Newton. Procaccini stated he was in the wrong lane and looked in his mirror before attempting to merge into the correct lane. Procaccini stated to the right of his vehicle he observed a vehicle "flying" towards the entrance to the Pike. Procaccini stated he came to stop to allow the vehicle to pass. Procaccini stated the vehicle passed at a high rate of speed and he continued on his way onto the Pike (E). Procaccini stated he had a passenger in the vehicle using Lyft servies and there were no signs that a crash had taken place. Procaccini's information has been added to his report to reflect him being the operator of MV2. I then spoke with Peter Houser who is a Driver Services Representative with Lyft. Houser stated he had just visually inspected the vehicle alleged to have been involved in the crash. Houser stated the vehicle was in "pristine" condition and he observed no damage to the front passenger side quarter panel of MV2. Houser e-mailed me pictures of the vehicle. I observed no damage to MV2. Those pictures were submitted to the IT Bureau.

RP/resident reports hearing a crash while inside his residence and looking outside and observed a silver SUV against the stop sign located at Derby St and Tolman St. RP reports MV in question left the area on Tolman St and then turned right onto Underwood Rd. RP states MV would have damage to drivers side and possibly the drivers side front quarter. A search of the area resulted in negative results. Traffic Officer McSweeney took photos of the damaged stop sign and the DPW was notified.

On Thursday 04/16/2020 at approximately 2306 hrs I was dispatched to a hit and run accident in the area of 320 Washington St. Upon arrival I observed debris all over the roadway including a City one way street sign, a City utility light pole along with a Metropolitan MWRA vent. The RP mentioned to dispatch he observed headlights driving away from the scene and believed it was a black vehicle once he exited his residence and observed the vehicle parts in the roadway.The vehicle later identified as a 2020 Toyota Rav 4 black in color bearing MA.REG (97E520) was not on scene of the accident. Shortly after I responded on scene the vehicle involved was located nearby by N-494 at the intersection of Washington St. at Adams St. The vehicle was registered to a Evgenii Shatokhin with heavy right front end damage along with the left front, and both rear tires deflated. Mr. Shatokhin stated he was the operator of the vehicle and I asked what happened back at 320 Washington St. He stated he was driving Westbound on Washington St. when he swerved to avoid something and hit the curb. Once he hit the curb, he stated he then hit the sign, light pole and MWRA vent. Due to Mr. Shatokhin not remaining on scene of the accident and leaving he is being charged with Ch. 90 Sec. 24 (Leaving the scene of an accident, property damage). Failed to remain on scene after accident occured involving property damage. He is also being charged with Ch. 89 Sec. 4A (Marked Lanes) for failure to drive entirely within. Citation #T2014768 was given in hand to Mr. Shatokhin. MV1 sustained heavy right front end damage. No injuries. Tody's responded on scene and towed MV1. A towed motor vehicle form was filled out and placed on file. Pictures were taken on scene of the property damage and placed into the IT Bureau's box.
