Narrative:
MA commercial reg M94754 (vehicle #1), a City of Newton pickup/sander truck, was located crashed over the curb in front of 1970 Beacon St. The vehicle was unoccupied, had the keys in the ignition, and was reported stolen just a few minutes prior to discovery. The reporting party of the stolen truck and uninvolved occupant is identified as City DPW worker George Sementelli. The witness/passerby who discovered the collision is also a DPW worker, identified as Donny Brandt. Pursuant to an investigation outlined in incident #19002213, the operator at the time of the crash was determined to be Michael Carter. It is believed that Carter stole the vehicle from 295 Auburn St, fled the area, and ultimately crashed in front of 1970 Beacon St. Corroborating facts outlined in the incident report put Carter as the operator. The vehicle itself was located partially on the sidewalk and partially on the road, therefore Carter is being cited with MGL 89/4A - Marked Lanes Violation (T1268423). Due to the fact it is believed that Carter fled the scene after the crash, MGL 90/24/C - Leaving the Scene of Property Damage was added as well as MGL 90/23/D Operating After a Suspended LIcense, and MGL 90/24/O - Reckless Operation of a Motor Vehicle. The crash occurred during busy weekday commuting about 0.7 miles from Angier Elementary School. The last 3 charges mentioned have been listed on a second citation (T1268422). Officer Brooks also issued T0648082 for OUI Drugs MGL 90/24/G. See the incident report for details about this charge. In total, there are (3) citations issued to Carter. The vehicle sustained heavy front end damage. It should be noted that the utility pole a couple of houses down from the crash site (Beacon/Varick Rd, pole #212) sustained fresh, minor superficial damage, consistent with the vehicle crashing into it before verring onto the sidewalk. Pictures have been taken and submitted to the IT bureau. Tody's responded and towed the vehicle to the police garage for processing due to the pending other criminal charges involved in this accident/incident. Michael Carter was subsequently placed under arrest and his citations have been placed into his property. Carter suffered a hand injury due to the crash an was transported to NWH Hospital for an evaluation after he was booked.

On the above date/time I responded to the area 2000 Washington Street Newton -Wellesley - Hospital for a two motor vehicle crash on the 3rd floor of the parking garage. Upon arrival I met with the operator of vehicle one, she stated she was pulling into a parking spot and applied the brakes but due to the ice on the roadway and slope of the roadway her vehicle slid into the rear of vehicle two which was parked. Vehicle two was unoccupied, and dispatch tried to get in contact with the owner to advise them. I also left a message with NWH in case the owner comes back in. No injuries and I advised of the process. Later in the day the owner vehicle two came to NPD, I gave him vehicle one inforamtion and advised him of the process.

MV#1 Stated while traveling westbound down Commonwealth ave he struck a patch of ice. The ice caused his vehicle to rotate 360 degrees and proceeded to slide on the roadside shoulder striking a verizon poll ( 102/285). MV#1 traveled approximately 12ft from the road before striking the poll. MV#1 sustained heavy front end damage. MV#1 had all airbags deployed and had a small fire under the front end. I observed multiple other vehicles passing having difficulties and vehicles sliding due to significant ice conditions. Driver of MV#1 had injuries to his left arm, possible broken forearm/wrist. Passenger of MV#1 stated she had chest pains. Medics transported both parties to Newton Wellesley Hospital for further evaluation. Tody's Towed the vehicle due to heavy front end damage.

Tipruthai Thangthong stated that on 05/21/2019 at approximately 16:36 hours she was pulling into the driveway of her sisters home at 53 Rangeley Rd while operating her vehicle MA reg 2LVL61 a 2018 Honda Accord. Thangthong stated that her three old son Mighty Mettanant was a passenger in her vehicle. Thangthong stated that she was driving approximately 5 MPH as she drove into the driveway. The driveway runs east to west and Thangthong's vehicle was heading west into the driveway as the sun was in the Western sky creating a glare. Thangthong stated that she did no see anything in the driveway prior to her pulling into it. Thangthong stated that she felt the front of her vehicle strike what she described as " something " and stopped her vehicle. Thangthong stated that she put her vehicle in reverse and then into park and exited her vehicle to see what she had struck. Thangthong stated that she saw her two nieces, Kalla Mongkhonlertsirkul age three and Kelley Mongkhonlertsirkul age seven laying in the driveway in front of her vehicle. Thangthong stated that she then realized that her vehicle had stuck her two nieces as they were in the driveway. Both of the Mongkhonlertsirkul sisters were conscious and alert and were able to walk to an ambulance for transport to Newton Wellesley Hospital for treatment. Thangthong stated that she and her son were not injured in the crash. I observed no damage to Thangthong's vehicle. I responded to the Newton Wellesley Hospital and spoke with the Mongkhonlertsirkul sisters and their mother Piyada Kellum. Kellum stated that her oldest daughter Kelly is a student at the Franklin Elementary School . Kellum stated that her daughters told her that they were playing in their driveway when their aunt's vehicle entered the driveway and did not stop. I spoke with both of the Mongkhonlertsirkul sisters who told me that they tried to get out of the way of their aunt's vehicle but could not in time. The Mongkhonlertsirkul sisters stated that they were both struck in the back and fell to the ground between the two front tires of their aunt's vehicle. Kalla Mongkhonlertsirkul had scrapes to her right knuckles. Kelly Mongkhonlertsirkul had lacerations to her lower left arm and the left side of her face near her eye. I spoke with hospital staff who stated that there did not appear to be any broken bones or other internal injures. I took photos of the crash area and forwarded the disk to the NPD's IT bureau for downloading.

On 6/7/2019 at approx 0827hrs while assigned to 497 I responded to 2014 Washington St, The Newton Wellesley Hospital for a report of a 2 car crash with possible injury. upon arrival I observed both FL Reg BWDG86 and MA Reg 3AT486 , both with major front end damage leaking fluids blocking the west entrance to NWH. Both operators were out of their vehicles. I spoke with Alicia LOWE who was operating FL Reg BWDG86 who stated she was travelling eastbound turning right into the parking lot when she collided with Helen COOK operating Ma Reg 3AT486 who was turning left into the parking lot from the west bound lane of Washington St. Both LOWE and COOK were complaining of pain but both signed refusals with Cataldo. Todys Tow responded and took both vehicles.

Operator of vehicle one, Benjamin Beaver stated that while driving south bound near 321 Walnut St his vehicle ( MA reg 551DK8 2014 Lexus ) made contact with a pedestrian in a marked crosswalk. Walnut St is a public way in the city of Newton. Beaver stated that he did not see the pedestrian prior to the crash and that his vehicle was travailing approximately 5 to 10 MPH. Beaver stated that the pedestrian was knocked to the ground but got up and walked around. The pedestrian, NNHS student Benjamin Donovan stated that he was crossing east to west in the marked crosswalk in front of 321 Walnut St. Donovan stated that he saw Beaver's vehicle approaching him at approximately 5 to 10 MPH. Believing that Beaver was stopping to let him cross , Donovan stated that he held up his left hand to signal to Beaver that he was going to cross. Donovan stated that Beaver's vehicle struck him on his left side and knocked him to the ground. Donovan stated that he had minor scrapes on his right side as a result of falling to the ground. Donovan stated that he did not lose consciousness and was transported via EMS to Newton Wellesley Hospital for treatment. I took photos of Beaver's vehicle and forward the disk to NPD IT Bureau. I issued Beaver MA Citation T 1269995 and cited him for a violation of MGL Ch 89 C 11, failure to yield to a pedestrian in a crosswalk. On 06/12/2019 I was forwarded an E-mail from Capt Anastasia of a video of the crash that was taken from a camera that was mounted on the roof of 321 Walnut St. The video shows Donovan walking into the marked crosswalk from the east side of Walnut St and hold up his left had to Beaver's on oncoming vehicle. Beaver vehicle appears to be travailing faster than the 5 to 10 MPH that Beaver believed that he was travailing. The video shows that Beaver did not brake until Donovan was struck and landed on his vehicles hood. Donovan was thrown approximately 10 feet from Beaver's vehicle and landed on his right side on the roadway. The video shows Beaver stopping and pulling to the side of the roadway after the crash.

Bicyclist, Tasker Howell, was traveling SOUTH on Watertown St, entering the lane of traffic when MV1 collided with her handlebars and front tire. MV1 (MA Reg: 8SP529, 2018 Ford SUV) was traveling WEST on Watertown St when she collided with the Tasker on the bicycle. MV1 operator, Maria Forgione, stated that while she was traveling on Watertown St, she had a passed a large landscaping truck parked at 857 Watertown St which hindered her seeing Tasker pulling out into traffic on the bicycle. Tasker stated the same, that she saw traffic clear and believed it would be safe to pull out into traffic but the landscaping truck created a blindspot. Tasker was not wearing a helmet. There was damage to MV1 on the passenger front end. Tasker was transported to Newton Wellesley Hospital. Tasker's bicycle was left in her possession at her residence at 871 Watertown St. Pictures were sumbitted to IT.

MV1 jumped a curb and collided with a parked vehicle MV2 (a 2004 GMC Yukon, MA Reg: 8EC792) in front of 185 Waltham St. MV1 operator, Angel Gonzales, was unconscious on scene in the driveway of 185 Waltham St. Waltham St is a public way that is owned and maintained by the City of Newton. A passerby, Joseph Anderson saw the aftermath of the crash and called 911. MV2 had damage to its drivers side door and rear side. MV1 was heavily damaged, on its side, and keys were in the ignition. MV1 was towed by towed by Tody's. Gonzales was transported to Beth Isreal in critical condition. When Gonazales was in the back of the ambulance, myself and medics smelled an odor of an alcoholic beverage emanating from Gonzales. All first responders thoroughly checked the surrounding area for a helmet but one could not be located. Due to Gonzales' critical condition and urgency to get to the hospital, and ID was not thoroughly looked for on his person. Ofc. Guarino, N491, retrieved an ID from Gonzales upon arrival at Beth Isreal Hospital. Ofc. Stake filled out a towed inventory form. Pictures taken and sent to IT. Sgt. Lee was notified and will be following up with this incident. MV1 operator, Angel Gonzales is being charged with the following (MA Uniform Citations T1442823 & T1442824): 90/24J - OUI Liquor (public way) 90/24E - Negligent Operation of a MV (operating recklessly at a high rate of speed) 89/4A - Marked Lanes 90/23 - Attaching Registration Plates (attaching registration plates to motorcycle to conceal the fact that it was unregistered. Registration plate on the motorcycle came back to a 2002 Honda VT750 motorcycle, MA Reg: 1T3988; VIN on the motorcycle came back to a 2005 Suzuki GSXR60, MA Reg: 1L4389) 90/9B - Unregistered MV (motorcycle 2005 Suzuki MA Reg: 1T3988 was confirmed unregistered by RMV) 90/23D - Operating MV with a Suspended License (RMV confirmed that Gonzales' license was suspended S94722414) (see Incident #19039806 for more details). For QED purposes, the charge 266/139 - Stolen Registration was added to this report because 90/23 - Attaching Registration Plates was not an option. Due to Gonzales' hospitalization, the citations will be mailed to his last known address. Officers were able to rise a Harold Curtis, the father of Anthony Curtis (owner of MV2, the GMC Yukon) at 185 Waltham St. He was notified of the incident and advised to call their insurance company. Curtis mentioned that he has cameras facing the street where the incident took place and will see if he can retrieve the footage for us. Police prosecutor to subpoena blood.

On 8/28/2019 at approx 1058hrs while assigned to 497 I responded to the area of 2014 Washington St for a report of a 2 car crash w/o injuries. Upon arrival I observed the vehicles on opposite sides of Washington St with NFD Lt Bourgeois standing by with his emergency lights activated, Lt Bourgeois stated both operators were shaken up but not injured. I spoke with the operator of Ma Reg 1XM680 Elizabeth CHISOLM who stated she was attempting to take a left onto Washington St, a vehicle in the right hand eastbound lane was turning right into the parking lot, CHISOLM stated she did not see Ma Reg EV3452 being operated by Julien GRANT in the left eastbound travel lane until it was too late colliding with him. I spoke with GRANT who stated he was traveling EB in the left travel lane of Washington St when struck by CHISOLM exiting the hospital.

Katerina Iliades stated that on 10/23/2019 at 11:56 hours she was standing in front of the rear driver's side door of her vehicle MA reg 7BM656 2019 Land Rover while it was parked facing north in front of 316 Walnut St. Walnut St is a public way in the city of Newton. Iliades stated that she was placing her daughter Melina Bichelen in her car seat and when she observed MA reg 8EP212 a white Subaru Outback driving north bound on Walnut St near the marked crosswalk at 312 Walnut St. Iliades stated that she observed the Subaru was being operated by an elderly white male. Iliades stated that the she observed the Subaru's front drivers side strike several orange plastic three foot poles that are affixed to the roadway around the crosswalk. Iliades stated that after the Subaru struck the poles at the crosswalk it veered towards her. Iliades stated that she leaned back into the back seat of her vehicle and pulled the door towards herself as the Subaru's passenger side mirror made conact with her vehicle. Iliades stated that the Subaru continued north bound on Walnut St without stopping or slowing. Iliades complained of lower leg pain and refused medical aid or transport to a hospital at the crash scene. I observed no damage or markings on Iliades vehicle. MA reg 8EP212 is registered to Harilaos Papadogiannis ( DOB 01/02/1941 ) of 134 Apt 4 North St , Newton. I went to 134 North St and found MA reg 8EP212 a 2014 Subaru parked in the lot of 134 North St. I observed that the Subaru's passenger side mirror was lose from it's mounting and was turned down and was facing the wrong way. I further observed that the out side casing of the Subaru's mirror had scuff marks consistent with it forcefully making contact with an other object. I spoke with Papadogiannis who stated that he was driving his Subaru north bound on Walnut St at 11:56 hours on 10/23/2019. Papadogiannis stated that he could only recall striking the plastic poles near the crosswalk and nothing else regarding the crash. I as spoke with Papadogiannis he was confused and I had to repeat myself and my questions to him several times. I instructed Papadogiannis what steps he needs to take regarding filing out an operator crash report and contacting his insurance provider , but I believe that he did not fully comprehend what I was conveying to him. Based upon my observations at the crash seen and my conversations with Iliades and Papadogiannis , I believe that Papadogiannis was at fault for the crash. Based upon my observations of Papadogiannis I filed an Immediate Threat Request form and faxed it to the MA RMV Driver Control Unit. Photos of both vehicles were taken and downloaded by the NPD's IT Bureau.

I was dispatched to 854 Beacon St, Newton, MA for an overturned motor vehicle accident. I arrived on scene and saw Vehicle #1 (MA reg 9AS213) overturned on its driver's side with the female operator/registered owner trapped inside. Newton Fire arrived on scene and mechanically removed the operator out of the motor vehicle. Prior to where Vehicle #1 was overturned I observed Vehicle #2 (MA reg 615FF6) with rear bumper damage and a snapped rear axle. Vehicle #2 was unoccupied. Operator of Vehicle #1 was transported to Newton-Wellesley Hospital for treatment by Newton Medics. Both motor vehicles were towed by Tody's and inventory sheets were completed and filed. I spoke with the Operator of Vehicle #1 at Newton-Wellesley Hospital. Operator stated that she does not remember what happened before the crash and that she only remembers waking up in her car. I told her that her car was towed and a report is on file. Ofc Selig spoke with the registered owner of Vehicle #2 and notified her of the incident. Based on my investigation it appears that Vehicle #1 struck the rear of Vehicle #2 and the operator over steered causing the vehicle to overturn onto the driver's side.

MV#1 was traveling North on Fair Oaks Ave, and attempted to make a right turn onto California St. MV#2 was parked in front of 589 California St facing West. MV#1 appeared to have run over the curb at the intersection, hit a city tree in front of 55 Fair Oaks Ave (California St side), and then crashed into MV#2. When I arrived on scene I observed a male party with blood on his head, and another male party standing next to him, later identified as Elmer Lopez.. I asked Elmer which car he was driving, and he stated that he was in MV#1. I asked Elmer what happened, and he stated that he and the other man with him, identified as Ricardo Deras, were in the rear seat with another male party, and that there were two other parties in the front seat, one of them was driving, he was not able to provide me with a name of who was driving. I asked him where everyone else was, and Elmer stated that after the crash, the got out of the car and ran away. At this point, medics arrived on scene and began to tend to Ricardo. I next spoke with the owner of MV#2, Jim Silva. Jim stated that he was in his home when he heard a loud crash. Jim stated that he came out of the house in time to witness the two males I was speaking with looking at the car, and he also stated that he observed three parties walk away from the scene, two heading east on California St, and one heading north on Fair Oaks Ave. MV#1 and MV#2 were towed off scene by Todys. Due to the scene, and the location of the cars in a busy street, I was not able to do a MV inventory of MV#1 until after it was towed by Todys. The inventory was done at Todys Service station, and a MV inventory sheet was filled out and placed the in appropriate box. I took three pictures of the damaged tree and submitted them to IT to be attached to this report. While I was taking the pictures of the tree, two males were walking past the scene. I stopped both parties and questioned them about the incident. They stated that they had been in the car, but had not been driving. I asked them if the knew who the driver was, and they told me that they did not know the name of the driver. Due to no witnesses observing the driver and all parties stating that it was someone else, I was not able to determine a driver of the vehicle. On 11/30/2019, while assigned to N494, I, Officer Conary, and other units, responded to Charlesbank Road at Nonantum Road for a single motor vehicle crash into a tree. Upon arrival, I observed a white mini van MA registration 5DV184 with heavy front end damage into a tree on Charlesbank Road, a public way in Newton. The operator of the motor vehicle, CORDERO, Ericka, was sitting in the back seat of the vehicle and was talking on the phone. CORDERO's primary language was Spanish. I asked CORDERO if she was okay and she said yes and I asked CORDERO if she was driving the car and she said yes. CORDERO also told me that she did not have a license. MA State Police were on scene and were able to asist with translating. Medics arrived and CORDERO was transported to Saint Elizabeth's Hospital. CORDERO's son and husband arrived on scene and I was able to confirm her information. Both stated that CORDERO did not have an active license. Newton Dispatch ran all information for CORDERO and confirmed she was unlicensed. CORDERO will be charged with MGL Ch.90/10 Unlicensed Operation of a Motor Vehicle and 89/4A Marked Lanes Violation, Uniform MA Citation T2079476. Due to CORDERO being transported to the hospital, the citation will be mailed to her active address. Pictures of the incident were taken and uploaded to IT accordingly. No further incident to report.

Operator of vehicle one, Christopher Sermak, stated that while turning north bound in the parking lot of 175 Boylston St he was blinded by solar glare. Sermak stated that the diver's side visor was down and the he was not wearing sun glasses. Sermak stated that he heard a loud bang coming from the front driver's side of his vehicle and realized that his vehicle had stuck something. Sermak stated that he stopped his vehicle and saw a male identified as John Baker laying on his back on the ground. Baker stated that he had step off the side walk in front of 175 Boylston St and was walking to his car when he saw Sermak's vehicle approaching him and realized it was not slowing, Baker sated that the front driver's side of Sermak's vehicle struck his right arm and shoulder causing him to be knocked to the ground on his back. Baker stated that his head struck the ground but he did not lose consciousness. Baker was transported to Newton Wellesley Hospital via EMS for treatment. While on scene I observed that the rear passenger side brake light of Sermak's vehicle was out. I issued Sermak MA Citation T 2012527 and cited him for violations of City of Newton City Ordnance 19-75, failure to use care while tuning , and MGL 90/7 , defective lights.

The operator of MV1, Nicole Laurin, sates that she was driving southbound on Sheridan Street when she lost control of her vehicle causing her to crash into MV2 which was parked in front of 107 Sheridan Street. Both vehicles sustained major damage and airbag deployment and were towed from the scene by Tody's. Due to the high rate of speed of the crash and air bag deployment, the operator of MV1 was transported to Newton Wellesley Hospital to be evaluated. It should be noted that prior to the reported crash there was a 911 call regarding a white sedan traveling at a high rate of speed going through numerous traffic signals and signs in a densely populated neighborhood with narrow streets. After the crash several more 911 calls were received stating that a vehicle traveling at an excessive rate of speed just crashed into a parked vehicle. Based on these calls and the nature of the crash, Ms. Nicole Laurin was issued in hand Massachusetts Unifrom Citation T2013231 for 90-24, Operating to Endanger, and 90-17, Speeding over the reasonable and proper rate. Sheridan Street is a public way in the city of Newton.

Sgt. McNeil, N495, and I responded to 474 Wodward St. for an auto pedestrian accident. Upon arrival the victim, Elizabeth Cantwell was laying in the street and receiving treatment from NFD and Cataldo. The operator of Ma Reg. 192HV0 was Margaret Meehan. Meehan was visibly upset and was hearing impaired, as a result Meehan was unable to communicate what happened in the accident. At this time we were able to contact Meehans daughter, Bernadette Conley 781-248-6222. Cantwell was transported to Beth Israel Hospital. Traffic Bureau was notified. I went to the BI hospital to get a statement from Cantwell. Cantwell states that her husband parked their vehicle across the street from Starbucks. Cantwell states that she stood next to her car to cross the street. Vehicle 1 came to a stop and Cantwell believed that the vehicle was going to let her cross the street (not in crosswalk). Cantwell states that when she went to cross the street vehicle 1 hit her, causing her to land on the hood and then rolled off onto the street. At this time Cantwell has a dislocated hip and head injury. I took photos of the scene. Meehans vehicle was parked in a legal spot and I transported her to NHQ where she was picked up by her daughter Bernadette Conley. I filled and immediate threat with the RMV and a copy of the report was attached with the form.

Operator of vehicle one , Allan Wint stated on 01/27/2019 at 09:40 hours he was driving his employer's pick up truck south bound on Lexington St near number 227. Lexington St is a public way in the city of Newton. Wint is a driver for Auto Zone auto parts located at 140 Lexington St , Waltham, MA ( 781 894-0656 ) . Wint was driving a 2018 Chevrolet Colorado pick up tuck ( MA reg 8CA823 ) that Auto Zone leases from Lease Plan USA Ltd. Wint stated that he was headed to Webster St in Newton to pick up a delivery. Wint stated that as he approached the area of 277 Lexington St he believed that the traffic light that is at the marked crosswalk was green for south and north bound traffic. Wint stated that he did not see any pedestrians in the crosswalk, until he heard the sound of an object impacting into the front driver's side of his vehicle. Wint stated that he stopped his vehicle approximately ten yards past the crosswalk and got out. Wint stated it was then he realized that his vehicle had stuck a female and a dog. Wint stated that he was not operating any electronic devices at the time of the crash, and I observed none in his vehicle. Wint stated he was not injured in crash. Wint's vehicle had damage to the driver's side mirror and lower molding of the driver's side front door. Wint's vehicle did not require a tow. Witness Bessy Shanks stated that she was driving in her vehicle behind Wint's vehicle south bound on Lexington St. Shanks stated that Wint's vehicle was driving slow and swerving approximately two blocks prior to the crash. Shanks stated that she believed that Wint may of been distracted. Shanks stated that south and north bound traffic was stopped because that cross walk light was red and that a female and a dog were crossing from the east to west in the marked crosswalk in font 227 of Lexington St. Shanks stated that the front driver's side of Wint's vehicle srtuck the female and the dog as they were approximately halfway into the crosswalk. Shanks stated that the female and dog both fell to the ground after the crash. Witness Elizabeth Keegan stated that she was driving in her vehicle south bound on Lexington St two vehicle behind Wint's vehicle. Keegan stated that she was not sure what color the traffic light was at the time of the crash, but traffic was stopped both not and south bound traffic was stopped. Keegan also stated that Wint's vehicle " didn't even slow down ". Keegan stated that Wint's vehicle struck the female pedestrian and the dog as they were in the marked crosswalk in front of 277 Lexington St. Witness Henry Taylor stated that his vehicle was the first one stopped at the red crosswalk light at 227 Lexington St in the north bound lane. Taylor stated that north and south bound traffic was stopped and that Wint's vehicle did not stop and stuck a female pedestrian and a dog in the crosswalk. Witness Jacqueline Deos stated that her vehicle was stopped behind Taylor's north bound on Lexington St. Deos stated that north and south bound traffic was stopped for the red light . Deos stated that Wint's vehicle did not stop and that it struck a female pedestrian and a dog in the marked crosswalk. The pedestrian in the crosswalk was identified as Xiaoping Sun. Sun stated that she and her dog stopped on the south bound side of the marked crosswalk across from 227 Lexington St. Sun stated that she activated the crosswalk light and waited until the light was red for north and south bound traffic, and that the walk single was activated. Sun stated that she looked to her left and saw traffic north bound traffic was stopping. Sun stated that she turned to her right and entered the marked crosswalk. Sun stated that she and her dog had crossed over the north bound side of Lexington St and she then looked to her right and saw Wint's vehicle not stopping and " going very fast ". Sun stated that the front driver's side mirror of Wint's vehicle stuck her in the face . Sun stated that as she fell to the ground she was struck on the left leg by Wint's driver's side. Sun stated that she did not lose consciousness after she had fallen to the ground. Sun was transported via EMS to The Brigham and Women's Hospital for treatment for head and neck injuries and abrasions to her legs. Sun's dog died as a result of being struck by Wint's vehicle. Photo's of the crash site and of Wint's vehicle were taken and the disc was downloaded by the NPD's IT Bureau. Before leaving the crash site, I tested the crosswalk lights and they were in working order. Based upon my observations of the crash site, Wint's vehicle, and the statements made to me, I concluded that Wint was at fault for this crash. I issued Wint MA Citation T 2013958 and cited him for a violation of MGL 89/11 , failure to yield to a pedestrian in a crosswalk.

While cleaning his car, he heard a bang, turned around, and observed the vehicles rolling off to the side. Witness stated he observed V1 and V2 in the middle of the road and not in the lane of traffic. Based on his observations he believes the collusion happened at the yellow lines. Witness stated he heard a car driving fast before the bang. He did not see the collision, but states V2 could have been driving too fast and tried passing on the left.

On 8/17/2020 at 1550 hrs, while on patrol, I was flagged down by a passerby stating a man had just fallen off his "Bike" in front of 1680 Washington St. Upon arrival, I observed a motorcycle parked on the side of the road with the operator standing next to it. The motorcycle had several scratches on its side and a cracked plastic window, consistent with a collision. The operator stated as he was traveling eastbound on Washington St. his motorcycle began to "skid" on the road's surface as he changed lanes. This "skid" caused the operator to loose control resulting in a collision with the road's surface. The operator had minor lacerations to the knee and elbow and was transported to Newton-Wellesley Hospital. The Motorcycle was placed in the adjacent parking lot, to be picked up later in the evening. It should be noted the roadway in front of 1680 Washington St. had recently been paved over and was smooth and free of debris or defect. There were yaw marks for approx 100 yards in both lanes. There was no indication from the marks of a lane change.

On August 19th, 2020, while working in marked unit N526, I responded to a MVA vs. bicyclist in the area of the intersection of Crafts Street and Watertown Street, Newton. The weather at the time of the crash was clear and cloudy. The road surface was dry. Crafts Street and Watertown Street are both public ways maintained by the City of Newton. The operator of MV1, Cody Hjelm (NHL19517660), stated he was traveling westbound on Watertown Street and had just crossed over Crafts Street when he saw a bicyclist traveling in the same direction on Watertown Street. As MV1 approached the bicyclist he moved over to the left towards the double yellow lines to give the bicyclist room. MV1 states the bicyclist was no longer in front of his vehicle when he heard a thud type noise come from the side of his van. MV1 stated he pulled over and saw the bicyclist on the ground with a head injury. MV1 sustained moderate damage to the right passenger side of the vehicle. The operator of MV1 reported no injuries and signed a patient refusal. MV1 was able to leave the scene of the accident. The bicyclist identified as Harriet Fell (S04980771) was being treated by medics on scene at the time of my arrival. I spoke with her briefly inside the ambulance as she was being treated for a head injury. Harriet stated she has no recollection of the accident and does not remember what happened before or after the crash. Medics advised she was being taken to NWH for treatment. I spoke with Harriet on the telephone around 1630 hours. She stated she still does not remember what happened but stated she was just finishing her forty mile bike ride and was heading home. Harriet stated she has a hematoma on her head and recieved three stiches. Harriet also stated she has small abrasions on both hands. Harriet stated she was being released from the hospital shortly and will pick up her bicycle tomorrow. The bicycle was transported to the police station by Newton DPW and was secured for safekeeping inside Support Services by Officer Manouk. A property form was completed and submitted along with an Incident report # 20039014. I took photographs of the accident scene and submitted them to the IT Bureau. Update by Ofc. Geagan: On 08/20/2020 Ms. Fell came to the Support Serices Bureau to retrive her bicycle. See attached Evidence/ Property Form and Return of Property Sheet

MV#1 is an MBTA bus. MV#1 sustained minor damages to its driver's side front bumper area. There were no reported injuries to the operator of MV#1 or its passengers. All of the passengers onboard of MV#1 left the area prior to my arrival except for 1. The operator of MV#2 stated she was travelling northbound on Walnut St when she was struck by MV#3, which in turn pushed MV#2 into MV#1. MV#2 sustained heavy front end damages with driver's side airbag deployment and heavy driver's side door panel damages. The operator of MV#2 sustained injuries to her neck and shoulders. She was transported by Newton Paramedics to Newton Wellesley Hospital. The operator of MV#2 had a small dog with her that was located in the front passenger seat. Newton Animal Control took possession of the dog and transport it to the veterinarian. It was reported that the dog sustained possible internal trauma. The operator of MV#3 stated he was pulling out of the driveway of Walgrens onto Walnut St and struck MV#2. The operator of MV#3 stated MV#2 was travelling at a high rate of speed. MV#3 sustained heavy front end damages. There were no reported injuries to the operator of MV#3. Tody's Towing took possession of MV#2 and MV#3. MBTA Police Officer Michael Bolotin arrived on scene and documented the accident (MBTA case #2020-5909). MBTA Officers took possession of MV#1. The City of Newton DPW's street sweeper cleaned the debris on the street.

MV1 stated they were traveling Northbound on Washington St. when an unknown MV also traveling Northbound crashed into the rear end of MV1 vehicle. MV1 stated the impact caused their vehicle to spin and knock over the St. James Circ. street sign. There was heavy damage to the rear of MV1. MV1 stated the vehicle that hit them was a blue pick up truck that fled the scene and countinued to the rotary at Newton Corner where they took a left onto Washington St. MV1 was unable to identify the driver or plate number associated with that vehicle. A canvass of the surrounding area was conducted for the blue pick up, yielding negative results. Tody's came and towed the vehicle. On Saturday January 2, 2021 at approximately 0115 hours I was dispatched to 229 Auburn St for a motor vehicle accident into a tree. MV1 was operating eastbound on Auburn St when it crossed over the marked center lane. MV1 struck the westbound side curb and jumped onto the sidewalk. MV1 then continued on the sidewalk until it struck the tree in front of 229 Auburn St. MV1 sustained total front end damage, as well as driver side damage. The driver side damage was to both driver side doors and both driver side tires were flat. MV1 was towed by Todys. MV1 operator was injured but conscious. However MV1 operator was unable to speak due to possible concussion injuries. MV1 was transported by Newton medics to Newton-Wellesley Hospital. MV1 was issued MA Uniform Citation T2015088 for marked lanes and speeding over reasonable and proper rate, and it will be sent to the MV1 operator via mail.

Operator of MV1 was traveling Westbound of Wachusett Rd. and collided with a utility pole in front of 48 Wachusett Rd. Operator of MV1 left the scene of the accident and was located nearby at the intersection of Hammondswood Rd. at Intervale Rd. Operator of MV1 stated he left the scene of the accident because he was scared. During questioning, Operator of MV1 admitted to consuming alcohol, appeared impaired and under the influence of alcohol on scene. Operator of MV1 completely left his lane of travel and was operating in a negligent manner. Operator of MV1 sustained injuries to his head and left ankle. He was then transported to Newton Wellesley Hospital for evaluation. MV1 sustained heavy front end damage due to impact. MV1 was unable to be towed due to live power lines underneath the vehicle. A voicemail was left with the traffic bureau to follow up on when it was safe to tow the vehicle involved. Operator of MV1 was mailed a criminal application, citation #T2012668 for the following offenses: 1. Ch. 90 Sec. 24/C (Leaving the scene of property damage). 2. Ch. 90 Sec. 24/E (MV, Negligent operation). 3. Ch. 90 Sec 24/J (OUI, Liquor). See incident #21016286 for further review.

On Friday, May 7, 2021, while assigned to Traffic unit N525, I responded to the area of Colby Road for a report of a crash involving a City of Newton owned vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with Mr. Michael Sanford from the City of Newton Water Department. Mr. Sanford stated his City of Newton owned vehicle (2013 Ford F-250 (MA MVN: M88493) was parked unoccupied (S) in front of 497 Centre Street, Newton. Mr. Sanford stated his emergency lights were on while he was marking out a service. Mr. Sanford stated he then observed a white 2005 Ford Ecowagon (MA CON: L61558) travel past his vehicle South on Centre Street and make contact with his driver side door mirror. Mr. Sanford stated the vehicle continued down Centre Street and he followed it to Rochester Road where is came to a stop. I observed damage to the driver door mirror. I spoke with the operator of MV2, Ms. Jerrie Spencer (S12241050). Ms. Spencer stated she knew she made contact with MV1's driver door mirror, but was trying to make it to her appointment at the Faulkner Hospital and that is why she did not immediately pull over. I observed damage to the front passenger side door mirror. Ms. Spencer was provided with Massachusetts Uniform Citation 994181AA for Chapter 89, Section 4A (Marked Lanes Violation). Centre Street is a public way maintained by the City of Newton. Photos were taken of both vehicles and submitted to the IT Bureau.

The OP. of MV#1 stated while traveling westbound on Washington Street, just passing Beacon Street, his vehicle suddenly struck a curb and he lost control of the vehicle. He ended up on the lawn of the Northland Building at 2150 Washington Street. Based on the accident scene, its appears that MV#1 went over the median on Washington Street, took down a city sign, hit the corner of Neshobe Road and the driveway into 2150 Washington Street which formed a ramp that launched his vehicle into the air. MV#1 spun around somehow and was now facing eastbound before the rear of the vehicle was wedged between a tree and a mound of dirt. Final resting position of MV#1 is approx. 250 feet from the median first struck. The operator of MV1 was transported to Newton-Wellesley Hospital for a head laceration and other unknown injures. Vehicle was towed from the scene by todys with major damage all over the vehicle. After reviewing surveillance video, the OP. of MV#1 was mailed MA. citation #T2014846 for violation of MGL Ch 89 Sec 4A - "Marked lanes violation" and Ch 90 Sec 17 - "Speed greater than reasonable."

On Thursday May 13th, 2021 at approximately 1353 hours myself and several units responded 55 Chapel Street for a motor vehicle that struck a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. Chapel Street is a public way owned and maintained by the City of Newton. Upon arrival Fire and medical personnel were attempting to remove the pedestrian from underneath the vehicle as her right leg was stuck in the undercarriage. Fire and medics were able to successfully free the pedestrian from the vehicle and render medical attention. The pedestrian was identified as Beth Herr ( S58524067) and it was determined that she was also the owner of the vehicle involved. I spoke to witness Joe Shields who stated he was outside his house located at 56 Chapel Street when his attention was brought to a blue SUV that was unoccupied moving backwards down Chapel Street. Joe stated he first witnessed the operator of the motor vehicle pull up to the UPS drop box located on the sidewalk across the street from his house. He stated he saw the female operator step out of the vehicle and drop off a package. Next he saw the vehicle start to roll backwards and the female operator opened the drivers side door and attempted to pull the emergency break. He stated the vehicle overtook her and she fell to the ground with her foot stuck under the car. Joe states he heard her yell and saw her get dragged by the vehicle to the opposite side of the street where the vehicle was stopped by the curb. Joe stated he tried to assist the woman but she was yelling in pain and he did not want to cause more injury so he waited for help to arrive. Witness Robyn Preston stated she was walking on Chapel Street when she saw the blue SUV rolling backwards with no one inside the vehicle. She states she then saw a woman try to stop the vehicle but she got dragged underneath the vehicle to its final resting place. Robyn stated she stayed on scene until help arrived. I was able to speak with Beth inside the ambulance prior to being transported to the hospital. Beth was conscious and alert but in a considerable amount of pain and discomfort. I asked her what happened to which she stated she drove her blue Toyota Rav4 MA REG: 1FHM53 to the UPS drop box located in the area of 55 Chapel Street. She stated she thought she put the vehicle in park but must not have as it started to roll backwards when she got out of the car. She states she tried to stop the car by opening the drivers side door and attempting to pull the emergency break. The car over took her and she was dragged to the ground with her right foot stuck underneath the vehicle. She was dragged across the street where the vehicle was finally stopped by a curb. Beth sustained minor scrapes and road rash on various parts of her body and her right leg was injured. Fallon medics transported her to the Brigham & Womens Hospital to be treated. Her husband was contacted using Beth's cell phone and notified of the accident. Captain Boudreau and I were able to drive her vehicle to her residence at 50 Broadway Street, Newton. Her vehicle was left parked outside her house and her car keys were left with her husband who was inside the residence. The vehicle did not sustain any damage. Pictures of the scene were taken and submitted to the IT Bureau. I will be following up with Beth at the hospital to check on her status. On Friday May 14th at 0900 hours I spoke with Beth on the phone. She stated she is home from the hospital and doing much better. She states she has right leg pain and road rash from the accident.

On Monday, May 17, 2021 I responded to the parking lot entrance of the Avalon at Chestnut Hill (160 Boylston Street, Newton) for a report of a motor vehicle/pedestrian crash. The weather at the time of the crash was clear and sunny. The road surface was dry. Boylston Street is a public way in the City of Newton. I spoke with the operator of MV1, Mr. Joshua Champion (SA1310472). Mr. Champion stated he was operating his 2013 Lincoln MKS (MA: VT41980) while attempting to exit the Avalon parking lot at the Boylston Street (E) off ramp. Mr. Champion stated he was monitoring the traffic to merge and as he started to make his right turn his vehicle crashed into the left side of a pedestrian in the roadway. Mr. Champion stated he did not see the pedestrian as he was making his turn. I observed no damage to the area where the pedestrian made contact with the car. Photos were taken and submitted to the IT Bureau. The injured pedestrian, Mrs. Joan Contompasis, stated she was walking on Boylston Street (W) with her husband. Mrs. Contompasis stated she entered the roadway from off the sidewalk to cross the Avalon parking lot exit and continue on Boylston Street. Mrs. Contompasis stated she believed MV1 had stopped to allow her and her husband to cross. Mrs. Contompasis stated the vehicle then accelerated and it's front passenger side bumper area crashed into her left side. Mrs. Contompasis fell to the roadway. Mrs. Contompasis was transported to Newton Wellesley Hospital for injuries to her right foot and knee. I spoke with a witness to the crash, Mr. Michl Contompasis. Mr.Contompasis stated he was walking with his wife Joan on Boylston Street (W). Mr. Contompasis stated he was walking just ahead of his wife as they crossed the Avalon at Chestnut Hill exit at Rt. 9. Mr. Contompasis stated he observed a black sedan to his left as he crossed. Mr. Contompasis stated he believed the operator was not paying attention as he exited the parking lot and the vehicle accelerated into his wife's left side.

On Monday, May 31, 2021 at approximately 00:40hrs I, Officer Daly, along with Officer Kayla Donahue responded to a motor vehicle accident. We found the vehicles on Washington Street by Park Street. Upon arrival I observed both of the involved motor vehicles pulled off to the side of Washington Street by Park Street. I spoke with the operator of MV1 who said he was stopped at the red light when he was hit from behind by MV2. I spoke with the operator of MV2 who said she was traveling Eastbound on Washington Street traveling approximately 20mph. Operator of MV2 said she began pressing down on her breaks due to the red light but said her car continued to slide and would not stop. Operator of MV2 said it was at this point her car collided with MV1. It should be noted it was raining throughout the day and it drizzling at the time of the accident causing the roadways to be wet and slippery. Both MV1 and MV2 were towed away from the scene by Tody's. Both of the operators retrieved their valuables from their vehicles prior to the vehicles being towed away. Both of the operators did not sustain any injuries. The passenger of MV1 began feeling ill while on scene and was transported to Newton Wellesley Hospital by Fallon Medics.

On Sunday June 13, 2021 at approximately 1933 hours I was dispatched to 2322 Washington for a two car MVA with no injuries. MV1 operator stated she was operating eastbound on Washington St (a public way) driving the speed limit. MV1 operator stated she observed MV2 on the westbound side of Washington St attempt to turn left into Starbucks (2322 Washington St). MV1 operator stated by the time she saw MV2 cut across in front of her and into her lane, it was too late and MV1 struck the passenger side of MV2. MV2 operator stated the same story. MV2 operator said she was also going the speed limit on Washington St westbound and attempted to turn left into Starbucks but misjudged MV1's speed, causing the MVA. MV1 sustained extensive total front end damage and was towed by Tody's. MV1 operator requested an evaluation by Newton Medics and was transported to Newton-Wellesley Hospital. MV2 sustained extensive and total passenger side damage and was towed by Tody's. MV2 operator reported no injuries.

On Monday, June 14, 2021 while assigned to Traffic unit N525, I responded to the intersection of Park Street and Alba Circle for a report of a motor vehicle crash involving a pedestrian. The weather at the time of the crash was overcast with light rain. The road surface at the location of the crash was wet. Park Street and Alba Circle are public ways maintained by the City of Newton. Park Street supports two way travel (N, S) with no marked divider to indicate travel lanes. I spoke with the operator of the 2013 Nissan Juke (MA: 137LX7) involved in the crash, Ms. Cynthia Cowan (S59504510). Ms. Cowan stated she was traveling on Park Street (N) towards Alba Circle. Ms. Cowan stated ahead of her there was a large landscaping truck (MA CON: X1515, 2001 Ford DRWSUP) parked on the right side of the roadway and blocking her lane of travel. Ms. Cowan stated she looked ahead of the landscaping truck and did not see any traffic coming towards her in the opposite lane. Ms. Cowan stated she entered the opposite lane of travel (S) to pass the parked landscaping truck and continue down Park Street. Ms. Cowan stated as she was traveling past the front of the landscaping truck, she crashed into a standing mower that had entered the roadway. Ms. Cowan stated the mower entered the roadway from in front of the parked landscaping truck. Ms. Cowan stated because of her obstructed view from the landscaping truck, she did not see the mower until it was too late. Ms. Cowan stated she stopped her vehicle and observed the operator of the mower on the sidewalk in front of 248 Park Street. I observed damage to the front passenger fender/bumper/wheel area with airbag deployment from the driver seat. I also observed yellow paint transfer on the front bumper area consistant with the color of the mower involved. MV1 was removed from the roadway by Tody's Towing. Ms. Cowan signed a patient refusal with Newton Medics and walked home from the crash site. The other party involved in the crash is a landscaper for 4 Seasons Services. I observed a yellow Wright Standing Mower 52" resting on the side of the roadway in the Northbound lane. The mower has significant damage to its front portion. 4 Seasons Services removed the mower from the roadway. A worker on scene estimated the value of the mower to be $11,000.00. Photos were taken of the mower, MV1, and the roadway and submited to the IT Bureau. The injured party, Mr. Louis Padilla-Gonzalez (S32927586), was transported to Newton Wellesley Hospital for a left shoulder injury and an injury to the left side of his face. I spoke with Mr. Padilla-Gonzalez in the emergency room with the assitance of a language translation service set up by the nursing staff there. Mr. Padilla-Gonzalez stated he was operating the standing mower and left the sidewalk into the road. Mr. Padilla-Gonzalez stated he drove by the front of the parked landscaping truck and when he got into the street he saw a car coming towards him fast. Mr. Padilla-Gonzalez stated he tried to reverse the mower and get out of the way, but he could not because of the wet roadway. Mr. Padilla-Gonzalez stated he was hit by MV1 and thrown from the mower to the street. After speaking with Ms. Cowan and Mr. Padilla-Gonzalez, it appears both parties had their view of the roadway obstructed due to where the 4 Seasons Services landscaping truck (MA CON: X1515) was parked. I did not observe any posted parking restrictions where the vehicle was parked, but due to the size of the vehicle it did take up the entire Northbound travel lane in front of 248 Park Street. The location of the parked landscaping truck and the wet road surface contributed to the crash. There were no witnesses to the crash.

Father stated he would take his son later by car to have his right forearm checked out because of airbag rash. On Monday, July 19, 2021, while assigned to Traffic unit N525, I responded to the area of Commonwealth Avenue and Lowell Avenue, Newton for a report of a MVA/Pedestrian crash. Commonwealth Avenue and Lowell Avenue are both public ways maintained by the City of Newton. The weather at the time of the crash was overcast with light rain. The road surface at the time of the crash was wet. Feeney Brothers Utilities has been performing gas repair services at this intersection for the past month and they have had a paid Newton Police Detail with them during their service. The detail presented itself as follows on my arrival. There was service being conducted in the northbound lane of Lowell Avenue in-between the Commonwealth Avenue carriage roads. A large trench was present in the northbound lane that made the roadway impassible. Traffic was permitted to travel down the southbound lane of Lowell Avenue towards Commonwealth Avenue for the detail. Access to Lowell Avenue from Commonwealth Avenue was prohibited. The northbound lane was blocked by orange construction cones and road closed signs. There was also no access to the Commonwealth Avenue carriage roads at this location. Both of these roadways were closed with orange construction cones and caution tape. There was also caution tape and orange construction cones restricting access to a walkway that traveled from Commonwealth Avenue to its carriage road just after Lowell Avenue (E). I also observed a "Sidewalk Closed" sign posted on the walkway. I spoke with Officer Gabriel of the Newton Police Department. Officer Gabriel stated he was working a paid detail for Feeney Brothers at this location. Officer Gabriel stated he had just assisted two female parties cross Commonwealth Avenue (E) at Lowell Avenue. Officer Gabriel stated a short time later, he heard a scream and observed two female parties laying on the roadway on the opposite side of Commonwealth Avenue in the carriage road. Officer Gabriel immediately contacted Newton Police Dispatch via his portable radio and requested aid for the injured parties. Prior to my arrival, Officer Gabriel stated one of the pedestrians was transported to Newton Wellesley Hospital with a head injury. I spoke with one of the pedestrians involved in the crash, Ms. April Stein. Ms. Stein stated she was walking in the carriage road of Commonwealth Avenue (E) towards Lowell Avenue with her friend Sarah Kish. Ms. Stein stated when they got to Lowell Avenue there was work being done in the roadway, and a Police Officer assisted them with crossing the area. Ms. Stein stated they then re-entered the carriage road when they crossed Lowell Avenue. Ms. Stein stated they were walking along the edge of the roadway when they were hit by a front loader. Ms. Stein stated they fell to the roadway due to the collision. Ms. Stein stated she did not hear or see the front loader backing up towards them and did not notice it because they were having an "intense conversation". Ms. Stein reported no injuries and signed a patient refusal with Fallon Medics. I asked Ms. Stein if she had transportation to her residence. Ms. Stein stated she would walk home. I spoke with the operator of the front loader involved in the crash, Mr. Mark Mannion (S91049325). Mr. Mannion is an employee of Feeney Brothers Utilities and stated he was operating a 2015 Case Loader (MA CON: R99118) at the time of the crash. Mr. Mannion stated he was backing up the loader down the Commonwealth Avenue carriage road (E) from Lowell Avenue. Mr. Mannion stated he was going to get a load of fill from a pile of dirt on the side of the road inside the marked off work area. Mr. Mannion stated as he was backing up, he was looking over his right should behind him to make sure there were no obstacles in his way. Mr. Mannion stated his emergency backing alarm was activated while he was backing up. Mr. Mannion stated he noticed something enter the roadway to his right side and immediately slammed on his brake. Mr. Mannion stated then heard a scream and saw 2 female parties on the ground. Mr. Mannion stated he exited the loader and checked on the welfare of the two women. Mr. Mannion's CDL License, Vehicle Registration, and Hoisting License are all active. I asked Mr. Mannion to put the vehicle in reverse to assess if the emergency backing alarm was functioning properly. Mr. Mannion was cooperative and while reversing the vehicle the emergency backing alarm was functioning properly in my presence. I asked Mr. Mannion if the loader had moved since it was brought to a stop due to the crash. Mr. Mannions stated it had not moved. I observed this vehicle to be within the parameters of the work zone that was taped off. The vehicle at this time was released back to Feeney Brothers. Photos were taken of the work site and loader and submitted to the IT Bureau. I responded to Newton Wellesley Hospital and spoke with Ms. Sarah Kish (S85088490) in the Emergency Room. Ms. Kish stated she was walking down the Commonwealth Avenue carriage road (E) towards Lowell Avenue with her friend April Stein. Ms. Kish stated a Police Officer assisted them with crossing Lowell Avenue via the crosswalk located on Commonwealth Avenue. Ms. Kish stated after they crossed Lowell Avenue, they walked to the carriage road. Ms. Kish stated as they were walking, she was hit from behind by a front loader in the roadway. Ms. Kish stated she did not hear or see the front loader and believes the bucket hit her in the head. Ms. Kish stated she fell to the roadway. Ms. Kish was transported to Newton Wellesley Hospital for her injuries. Ms. Kish stated at this moment, she had some lacerations to her back and head, and her left shoulder/neck/back area was sore. I asked Ms. Kish how often she walks that particular route. Ms. Kish stated 5 times a week. Ms. Kish stated there was nothing in place to restrict her access down to the carriage road after she crossed Lowell Avenue. I spoke with a witness to the crash via telephone, Ms. Megan Boitano. Ms. Boitano stated she was traveling on Lowell Avenue (N) towards Commonwealth Avenue. Ms. Boitano stated she was stopped at the light right next to the trench that was in the northbound lane of Lowell Avenue. Ms. Boitano stated she observed two women crossing Lowell Avenue at Commonwealth Avenue in front of her. Ms. Boitano stated she then observed a heavy piece of construction equipment approaching the trench to her left at a high rate of speed. Ms. Boitano stated it startled her as the front loader dumped dirt into the trench. Ms. Boitano stated she then observed the front loader back down the carriage road at a high rate of speed towards a dirt pile. Ms. Boitano stated she observed the two women that just crossed Lowell Avenue in the roadway behind the front loader. Ms. Boitano stated she lost sight of the women, and then observed the front portion of the front loader aggressively shake. Ms. Boitano stated she then observed one of the women walk around from the side of the front loader holding her head. As presented while investigating this crash, the Feeny Brothers work site appeared to be properly set up to mitigate a potential safety incident with ample cones, barriers, caution tape, and signage. There was also a police detail present to assist with pedestrian travel. The area of the work zone where the pedestrans entered from Commonwealth Avenue after crossing Lowell Avenue was taped off with caution tape and a sidewalk closed sign was present. The crash happened inside the work zone parameters.

Operator of vehicle one Newton FF Eric Howard stated that while getting into the drivers seat of Newton Medic Unit 3 a Fallon Ambulance ( 2021 Ford Econoline MA reg AMN 7715 ), that was parked in front of 19 Faxon St, he inadvertently stepped on the break and disengaged the gear shift so that the ambulance went from park to drive. FF Howard was getting ready to drive Newton Medic Unit 3 to Newton Wellesley Hospital while Fallon Medics attend to their patient in the rear of the ambulance. FF Howard stated that after the ambulance went into drive it drove south bound on Faxon St and it's front driver's side fender struck the rear passenger side bumper of vehicle two. FF Howard stated that he was not injured in the crash and was able to drive the ambulance to Newton Wellesley Hospital. Fallon Medics Jared Gaglio and Shane McMahon stated that they were in the rear of the ambulance treating their patient, Dominica Tramontozzi when they felt the ambulance move forward and strike vehicle two. Both Gaglio and McMahon stated that they were not injured in the crash. I was unable to take a statement from Tramontozzi. Operator of Vehicle two Newton FF Joseph Colella stated that he was sitting in the driver's sea of Newton Fire Dept Eng 8 ( 2004 E-One Pumper MA reg MF471 and Newton Fire LT Douglas Quinn was sitting in the front passenger seat when the crash occurred. Vehicle two was parked in front of 19 Faxon St. FF Colella stated that the front driver's fender of vehicle one stuck the rear passenger bumper of vehicle two. Both FF Colella and Fire LT Quinn stated that they were not injured in the crash. Vehicle two did not require a tow. Vehicle one was towed from Newton Wellesley Hospital after a tow was arraigned by a Fallon Ambulance Supervisor. I took photos of both vehicles and forwarded the disk to the NPD's IT Bureau for downloading. Faxon St is public way in the City of Newton.

On Wednesday, August 4, 2021, while assigned to marked unit N493, I responded to the area 2115 Commonwealth Avenue, Newton with several other units for a report of a motor vehicle crash with entrapment. Commonwealth Avenue is a public way maintained by the City of Newton. The weather at the time of the crash was clear and sunny, The road surface was dry. On arrival, I spoke with the operator of MV1, Mr. Robert Bemis (S57110680). Mr. Bemis stated he was operating his 2019 Nissan Murano (MA: 1WHT71) on Commonwealth Avenue (W) towards Melrose Street. Mr. Bemis stated a red SUV traveling on Commonwealth Avenue (E) entered his travel lane and he had to swerve to avoid crashing head on. The front driver side of MV2 crashed into the driver side of MV1. I observed moderate damage to the entire length of MV1's driver side. Mr. Bemis reported no injuries and signed a patient refusal with Fallon Medics. Mr. Bemis was able to operate his vehicle away from the area. I spoke with the operator of MV2, Mr. Robert Shea (S86188377). Mr. Shea stated he was operating his 2011 Chrysler Town and Country (MA: 2LYT84) on Commonwealth Avenue (E) towards Lexington Street. Mr. Shea stated he was attempting to make left turn towards Lexington Street when MV1 hit him. Mr. Shea was transported to Newton Wellesely Hospital for evaluation. I observed signifcant damage to the front driver side of MV2. MV2 was removed from the roadway by Tody's Towing. The point of impact of the crash occured in the Westbound lane of Commonwealth Avenue. While traveling Eastbound on Commonwealth Avenue, MV2 crossed the double yellow unprotected median on the roadway and entered the Westbound travel lane. MV2 applied his brakes leaving skid marks to the point of impact and crashed into the driver side of MV1. The operator of MV2 will be cited with Massachusetts Uniform Citation 090421AB for Chapter 89, Section 4a (Marked Lanes Violation).

On Sunday, August 22, 2021, I was contacted by Sgt. M. Wade of the Newton Police Traffic Bureau at 0032 hours. Sgt. Wade stated a serious motor vehicle crash occurred on Washington Street in Newton and it involved a City of Newton marked police cruiser (N494). Sgt. Wade stated Officer Katelyn Pohlman and the operator of the other vehicle involved were transported to the hospital with potential serious injuries. I was requested by Sgt. Wade to respond to the scene from my residence and investigate the crash further. On arrival at the scene, I observed the two vehicles involved in the crash to be a 2015 Ford Explorer (MA: 586RT7) and Newton Police marked unit N494 (MA MVN: MP494B). These vehicles were observed disabled on the roadway in front of 612 Washington Street, Newton. There was a large debris field comprised of various parts from both vehicles on the roadway and in the parking lot of 612 Washington Street. The road surface at the time of my arrival was wet and there was light rain falling. Sgt. Wade reported that at the time of the crash the road was completely dry and it had not started to rain yet. Washington Street is a public way maintained by the City of Newton. The portion of Washington Street where the crash took place supports two- way travel, Eastbound and Westbound. There are two travel lanes in both directions. Travel direction is divided by a double yellow line in the middle of the roadway. The area is well lit and features numerous light poles illuminating the roadway. The closest intersection to this location is Crafts Street just west of the crash location. MV1 (MA: 586RT7) had substantial front-end damage, especially to its front passenger side and was inoperable. I observed this vehicle at its final rest on the roadway with its front end facing Northbound on Washington Street just over the double yellow line in the middle of the roadway. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV1 was removed from the roadway by Tody's Towing. MV2 (MA MVN: MP494B) had substantial front end damage and was inoperable. The impact from the collision caused the vehicle to spin uncontrolled and the final rest, with the front end of the vehicle facing the front entrance to the 7-11 (S). The vehicle's final rest was on the sidewalk in front of 612 Washington Street. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV2 was removed from the roadway by Tody's Towing. The Officer involved in the crash is Officer Katelyn Pohlman. Officer Pohlman stated she was operating Newton Police marked unit N494 on Washington Street (E) past Crafts Street. Officer Pohlman stated as she was operating her vehicle past 612 Washington Street (7-11), a vehicle in the westbound lane of Washington Street abruptly crossed the double yellow line in the roadway and entered her travel lane head-on. Officer Pohlman stated she did not have an opportunity to react to the vehicle in front of her and the front passenger side of MV1 crashed into the front end of her vehicle (MV2). Officer Pohlman sustained injuries to her left forehead area and both of her arms. Officer Pohlman was later transported to Brigham and Women's Hospital for evaluation and was released a short time later. It was reported to me by numerous Officers on scene that Officer Pohlman exited her cruiser after the crash and rather than tend to her own injuries, she focused her efforts on rendering aid to the operator of the other vehicle involved. Officer Pohlman found the operator of the other vehicle lying on the floor of the front passenger seat under the dash. Officer Pohlman rendered aid to the operator while waiting for Newton Medics to arrive. Officer Pohlman was also assisted in rendering aid to the other operator by Officer Conary and Officer Larson. Officer Larson reported that while interacting with the operator of MV1, the operator repeated numerous times, "Is she ok? Is she ok?". Officer Larson also stated the operator said to him "I feel bad because I was texting on my phone". Officer Larson stated the operator of MV1 works for the Department of Corrections Special Operations Unit and has Department issues equipment inside the vehicle. The following items were placed into Support Services, locker 9 for safekeeping by Officer Larson: 1 Gray Pelican Gun Case, 1 Black Gun Belt, 1 Taser, 1 Can OC Spray, 15 (50) Round Cases Of Ammunition. The operator of MV1 (MA: 586RT7) was identified as Mr. David Cafarelli (S16965940). Mr. Cafarelli was transported to Newton Wellesley Hospital for a potential head injury before my arrival. I spoke with Mr. Cafarelli at Newton Wellesley Hospital in the Emergency Room with Sgt. Wade. Mr. Cafarelli's mother and father were present when we spoke with him. Mr. Cafarelli stated he was out with friends before the crash and was driving to the 7-11 on Washington Street. Mr. Cafarelli admitted that before the crash, he had his cell phone in his hand while he was operating his vehicle. Mr. Cafarelli stated he does not remember if he was texting or changing the music on his phone at the time of the crash. Mr. Cafarelli stated he looked up and was unable to avoid the vehicle (N494) directly in front of him. Mr. Cafarelli stated he also does not believe he applied his brakes before the collision. When we spoke with Mr. Cafarelli in the Emergency Room, he was still waiting for test results to determine the severity of his injuries. On Monday, August 23, 2021, Sgt. Wade responded to 612 Washington Street, Newton to retrieve surveillance footage from the 7-11 of the crash. 7-11 stated their outside surveillance system was not operating correctly, but they were able to capture some of the incident from a camera inside the store. I observed this footage. MV1 is observed traveling on Washington Street (W) in the westbound travel lane in front of the storefront. MV1's driver side is then observed traveling over the double yellow line in the middle of the roadway and into the eastbound lane of Washington Street. A head on collision ensues and MV1 can be seen rotating counterclockwise in the roadway backwards to its final rest location on Washington Street. MV2 is then observed at its final rest position on the sidewalk facing southbound in front of the 7-11 storefront. After completing my investigation, I have surmised the following. Mr. Cafarelli was operating his 2015 Ford Explorer (MA: 586RT7) in the area of 612 Washington Street, Newton. Mr. Cafarelli was holding his cell phone in his hand while operating his vehicle. Mr. Cafarelli became distracted while he was operating his vehicle due to his attention being focused on his cell phone. Mr. Cafarelli traveled over the double yellow line from the westbound lane of Washington Street and entered the eastbound lane of Washington Street. Newton Police marked unit N494 was traveling on Washington Street (E) in the area of 612 Washington Street at this time. Mr. Cafarelli's vehicle crashed into the front end of Newton Police marked unit N494. Mr. Cafarelli was not wearing a seatbelt and as a result of the impact from the crash, he was ejected from the driver seat and his final rest inside the vehicle was in the flood of the front passenger seat. As a result of my investigation, Mr. Cafarelli was issued Massachusetts Uniform Citation for Chapter 90, Section 13B (Hands-Free Violation), Chapter 89, Section 4a (Marked Lanes Violation), and Chapter 90, Section 13a (Seat Belt Violation). Photos from the crash scene and the surveillance media from 7-11 capturing the crash were submitted to the IT Bureau.

Operator of MV1 was evaluated due to airbag deployment and her father signed a AMA refusal, stating they would take her on their own to the hospital. The weather at the time of the crash was cloudy was light rain. The road surface was wet. Washington Street is a public way maintained by the City of Newton. The operator of MV1, Fanes Nerestant (S47503048) stated he was operating a City of Boston owned 2012 Chevrolet School Bus (MA SBN: SB37810) west on Washington Street towards the Learning Prep. Mr. Nerestant stated he slowed his vehicle down and put on his right direction to take a right turn into the parking lot at 1488 Washington Street to pick up students at the Learning Prep School. Mr. Nerestant stated he then felt impact from the back of the bus and heard a "boom". Mr. Nerestant was transported to Newton Wellesley Hospital for neck and back pain. I observed minor damage to the rear driver side bumper/fender area. There were no other occupants on the bus at the time of the crash. The operator of MV2, Mr. Allen Simon (S44212695) stated he was operating his 2018 Nissan Altima (MA: CS12BM) west on Washington Street near 1488 Washington Street. Mr. Simon stated the school bus in front of him in his travel lane abruptly stopped to make a turn and he was unable to avoid crashing into the vehicle. MV2's front passenger side front end sustained heavy damage as a result of the collison. Mr. Simon reported no injuries. MV2 was removed from the roadway by Tody's towing. Mr. Simon stated his vehicle had a camera in it at the time of the crash and Mr. Simon stated he would forward the footage of the crash to me.

Mr. Doughty stated a vehicle in front of him came to quick stop in the roadway and was attempting to make a left turn into the Stone Institute located at 277 Elliot Street. Mr. Doughty reported back pain as a result of the crash and was transported to Newton Wellesley Hospital by Fallon Medics for further evaluation. Officer Durickas observed damage to the rear bumper area of MV1. A private tow was contacted by Mr. Doughty and his vehicle was removed from the roadway. The operator of MV2, Mr. Denis Doucet (S15405545), stated to Officer Durickas he was operating a City of Newton owned 2018 Ford F350 (MA MVN: M2954A near 277 Elliot Street (N). Mr. Doucet stated the vehicle in front of him came to a quick stop and the front end of his vehicle crashed into the rear of MV1. Mr. Doucet reported no injuries. Officer Durickas observed damage to the front bumper/front passenger side fender area of MV2. Mr. Doucet is employee d by the City of Newton DPW. Photos were taken of both vehicles and submitted to the IT Bureau.

On Thursday, September 23, 2021, while assigned to Traffic unit N525, I responded to the area of 358 Beacon Street, Newton for a report of a motor vehicle/bicycle crash with injury. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of the vehicle involved, Mr. Thomas Mogan (S29907615). Mr. Mogan stated he was operating his 2015 Jeep Wrangler (MA: 6EBH20) on Beacon Street (W) towards his residence of 358 Beacon Street. Mr. Mogan stated he observed a break in the traffic traveling on Beacon Street (E) towards Hammond Street in front of his driveway. Mr. Mogan took a left turn from the Westbound lane of Beacon Street to enter his driveway. While making his turn, his vehicle crashed into a bicyclist traveling on Beacon Street (E) in the marked bicycle lane in front of 358 Beacon Street. Mr. Mogan stated he did not see the bicyclist until the front passenger side fender/door area of his vehicle made contact with her. I observed minor damage to this area on his vehicle. Mr. Mogan reported no injuries. The injured bicyclist was identified as Ms. Louisa Palmer (S67185637). Ms. Palmer was treated on scene by Fallon Medics for injuries to her left wrist and ankle and transported to Brigham and Women's Hospital for further evaluation. Ms. Palmer was wearing a helmet at the time of the crash and I observed her blue Bianchi Pista bicycle resting on the sidewalk in front of 358 Beacon Street. The bicycle had minor damage in to it's front wheel area and fresh scrapes near the handlebars. I transported the bicycle to Ms. Palmer's residence. I responded to Brigham and Women's Hospital in Boston and spoke with Ms. Palmer in the Emergency Department. Ms. Palmer stated she was riding her bicycle on Beacon Street (E) towards Boston College. Ms. Palmer stated she was riding in the marked bicycle lane with the motor vehicle traffic to her left. Ms. Palmer stated she observed a break in the traffic on Beacon Street ahead of her, and then a Jeep entered her travel lane after it took a left turn from the Westbound travel lane on Beacon Street right in front of her. Ms. Palmer stated her left side crashed into the front passenger side wheel/hood area. Ms. Palmer stated she then fell to the roadway. Ms. Palmer stated she has injuries to her left wrist/arm area, her left ankle, and her hips. I spoke with a witness to the crash, Ms. Kelli McCusker. Ms. McCusker stated she was traveling on Beacon Street (E) towards Boston College at the time of the crash. Ms. McCusker stated there was heavy traffic in the area as she observed a bicyclist travel by the passenger side of her car. Ms. McCusker stated there was a break in the traffic ahead of her in the Eastbound lane and she observed a Jeep in the Westbound lane of Beacon Street make a left turn in front of the car in front of her. Ms. McCusker stated the Jeep and the bicyclist crashed into each other at this time. After speaking with all parties involved, Mr. Mogan did not yield to the right of way while Ms. Palmer was traveling on Beacon Street (E) on her bicycle. Mr. Mogan will be issued Massachusetts Uniform Citation for Chapter 90, Section 14B (Fail To Yield To Bicyclist on Turn). Photos were taken of the scene and submitted to the IT Bureau.

I responded to 234 Langley Road for a two car M/V crash with entrapment. Upon arrival with other officers, Fire, and medics I saw the following. MV1 was flipped over on the road way and the operator was still in the vehicle, but the passenger was outside the vehicle. MV1 Hit a parked MV2 unoccupied. MV1 was traveling north on Langley Rd when MV1 swerved to its right and made contact with the left rear bumper of the MV 2. The impact of the two MV's caused MV1 to flip over and land on its roof. Based on my observation the scene indicated : day light, clear and there were no road defects. I took numerous pictures which were submitted to IT bureau so they can be attached to this report. Both MV1 driver and passenger were taken to Beth Israel Hospital in Boston. Tody's arrived on scene and took possession of both MV's. A canvas of the area resulted with one neighbor at 255 Langley who witnessed the accident. Mr. Vish Showdhary stated that he saw MV1 driving on Langley Rd when MV1 crashed into the parked MV. He stated that there were no other vehicles on the road and that MV1 drove right into the parked vehicle. The scene was cleaned and I went to Beth Israel Hospital to speak with the operator and passengers of MV1. The operator of MV1 stated that they have no recollection of what happened. She stated that she does not know how she hit the parked MV and that the last thing she remembers is being pulled out of the rolled over MV by the Fire Department. The passenger in MV1 stated that he does not remember how the crash occurred. He stated that he was unsure how MV1 crashed into MV2. He stated thathe was able to get out of the MV without a problem. After a review of the crash I am citing MV1 with the following. M.G.L 89/4A Marked lanes and M.G.L 90/24E Negligent Operation.(Citation # 1445881) Furthermore a request for immediate threat license revocation was filled and submitted to the RMV. The MV Citation was delivered to the operator of MV1 in hand at the Beth Israel Hospital.

Operator of vehicle one ( 2019 Honda CRV MA reg 2SAH91 ) Elizabeth Sylvia stated that on 11/16/2021 at 16:56 hours she was driving south bound on Needham St near Charlemont St. Needham St is a public way in the City of Newton. Sylvia stated that as her vehicle slowed to turn left onto Charlemont St it's front driver's side struck a pedestrian in a crosswalk. Sylvia stated that she did not see the pedestrian prior to the crash due to the area not being lit. It should be noted that the over head street light was inoperable, and the cross walk was not illuminated or reflective. The crosswalk sign that was posted before the crosswalk was not reflective. Sylvia stated that she was unaware that a crosswalk was at the crash location. Sylvia was not injured in the crash , and her vehicle had no damage to it. The pedestrian Nivicha Morpeau stated that she was in the crosswalk in front of 241 Needham St when she was stuck by the front driver's side of vehicle one. It should be noted that Morpeau was wearing dark clothing at the time of the crash. Morpeau complained of back pain, but refused medical treatment and transportation to the hospital. Morpeau singed a patient refusal form provide to her by Medics. Based upon statements made to me by Sylvia and Morpeau I issued Sylvia MA Uniform Citation T 2016746 and cited her for a violation of Newton City ordnance 19/116 failure to stop for a pedestrian in a crosswalk. I took photos of vehicle one and the disk was forward to the NPD's IT Bureau for downloading.

MV1 operator stated he was operating westbound on Watertown St by Dunstan St (both public ways). MV1 operator struck (side swiped) both the driver side's of MV2 and MV3 which were parked in front of 915 Watertown St. MV1 operator struck the curb in front of 919 Watertown St and left the roadway. After MV1 left the road way it hit and destroyed a fire hydrant owned and maintained by the City of Newton. MV1 operator then hit the private wall of 919 Watertown St and it was it's final resting location. MV1 sustained extensive and complete front end and under carriage damage, it was towed by Todys. MV2 sustained minor damage and paint transfer, no tow required. MV3 sustained extensive driver side wheel well damage, which will privately towed at a later time. MV1 operator stated he struck his head and was not feeling well; he was transported to Newton-Wellesley Hospital by Newton Medics. MV2 and MV3 were not occupied. The City of Newton was notified in regards to the fire hydrant.

At approximately 1425HRs on Wednesday, November 24, 2021, I was dispatched to a motor vehicle crash, which took place on Nahanton St., between number 293 and Winchester St. Upon arrival, I observed MV1 to be facing eastbound in the westbound travel lane. It had its passenger's side front bumper contacting the passenger's side front bumper of MV3 and MV2 was off of the roadway, to the left of MV1's front driver's side. MV1: I noted damage to its front driver's side bumper, head light, fender, hood area and flat tire. On MV1's front passenger's side, I noted damage to its bumper, head light, fender and hood area. MV2: I noted damage to its front driver's side bumper, head light, fender, hood area and flat tire. MV3: I noted damage to its front passenger's side bumper. Upon speaking with the operator of MV1, they stated they were traveling eastbound on Nahanton St. At this time, they began to retrieve something from their bag on the passenger's seat. They then crossed MV 1 over the center yellow divider lines and struck MV2 and MV3. Upon speaking with the operator of MV2, they stated the last thing they remember is that they were traveling westbound on Nahanton St. and then that their vehicle was traveling backwards; They do not remember the crash. Upon speaking with the operator of MV3, they stated they were traveling westbound on Nahanton St. when MV1 crossed over the center yellow divider lines at a high rate of speed. They observe MV1 crash into MV2, pushing it backwards and to the side. MV1 then continued forward and impacted MV3. The operator of MV1 received a ride from their father, after signing a patient refusal. Newton Medics transported the operator of MV2 to the hospital. And the operator of MV3 was able to drive their vehicle away, after signing a patient refusal. Both MV1 and MV2 were removed by the scene by Todys Services (NPD Towed Motor Vehicle Form Filled). I cleared without further incident.

Vehicle #1 was traveling northbound on Centre St. at approximately 25 MPH when they witnessed someone enter the roadway on foot from between two parked cars and attempted to cross the road. Vehicle #1 stated they attempted to stop but was unable to do so and struck the person with the front left corner and windshield of the vehicle. The pedestrian and owner of Vehicle #2 stated he parked his vehicle on the right side of the road and exited the car. He stated he was walking towards the crosswalk and attempted to cross the street from the front of his vehicle. All he remembers is taking a few steps out into the road, being hit, and flying before hitting the ground. He was unaware where he was struck from. Vehicle #1 had moderate damage to the front and windshield and was towed by Tody's towing. Vehicle #2 was uninvolved in the crash and was parked at the time. The owner of Vehicle #2 and pedestrian struck was injured and transported by Medics to Brigham and Womens Hospital for injuries to his head, left arm, and left leg. Operator of Vehicle #1 stated he was uninjured and refused any medical treatment. It should be noted that the accident happened outside the crosswalk and the street lights above where the accident occurred were not working at the time of the crash making the roadway dark and visibility difficult. Photos of the accident were taken and saved on a memory card.

On 01/03/22 at approximately 1617 hours Officer Robert March and I, Officer Whitney Hyde, were assigned to N499 when we were dispatched to a 2 car MVA at 160 Needham St with no injuries. Both vehicles were waiting in the parking lot. On location I made contact with the operator of Vehicle 1, identified as Samantha LERNER. LERNER stated that the had been travelling southbound on Needham St and stopped in the center left turn lane to turn into the Farm Grill. When it was clear to turn, LERNER began to turn left when Vehicle 2 passed her on the left and made contact with the driver side front quarter panel of Vehicle 1. I observed scrapes and minor damage to Vehicle 1's front bumper and driver side front wheel rim. LERNER stated that the operator of Vehicle 2, later identified as Leanna LANDE, exited her vehicle at the time of the accident and exclaimed that her dog was dying and she was headed to the animal hospital down the road and they could exchange information there. LERNER proceeded to the animal hospital and was waiting for LANDE to come back out. I then entered the animal hospital and made contact with LANDE. LANDE was extremely upset as her 7 week old puppy had just died. LANDE stated that she had been in a panic because of her dying dog and passed LERNER when she shouldn't have, stating it was "stupid." Outside of the animal hospital I observed Vehicle 2 had minor damage along the length of the passenger side, including scrapes with white paint transfer. I issued LANDE MA Uniform Citation #T1448086 in hand for 89/4A Marked Lanes violation and explained the payment and appeal process. LANDE stated she understood. I provided LANDE and LERNER with the accident number and facilitated their exchange of information before clearing without further incident.

On Tuesday, February 15th, 2022 at approximately 1908 hours the operator of Vehicle 1 was travelling Westbound on Auburn St when it crossed over the center double yellow lines, crossed the opposing lane of traffic, and went off the roadway before crashing through the vinyl privacy fence at #106 Auburn St. Vehicle 1 came to rest after colliding with the North-East corner of the residence. The operator of Vehicle 1 then attempted to reverse away from the crash and backed into and destroying more of the residence's privacy fence. The operator was then confronted by the occupants of the residence, including the homeowner. Fire and EMS arrived on scene and conducted patient care on the operator and formed the opinion that she was intoxicated. I arrived and interviewed the operator who stated that she had previously taken oxycodone. The operator was then transported to Newton Wellesley Hospital. The vehicle was towed from the scene by Tody's. The operator was charged with the following violations: C90ss24 OUI- Drugs C90ss24 Neg Op C89ss4A Marked Lanes An incident report was completed for this crash (See Report #22005833).

Bicycle was traveling either north on Walnut Street or west on Washington Street from the area of Newton Centre. MV1 pulled out from a side street, unknown which side of street, in front of the Bicycle. Causing bicycle and vehicle to collide in an unknown manner. Further causing the bicycle and operator to flip forward in the air. MV1 left the area. MV1 is an unknown description. Operator of bicycle continued on foot on his path home where he was picked up by his father and brought by his father to NWH for cuts on his face and hands. Operator of Bicycle was disoriented and stated he did not know where the accident occurred, manner of collision, MV1's description or if he was wearing a helmet. Operator of bicycle left the bicycle on the sidewalk or in the bushes somewhere on his route home. Bicycle was described as a light blue Trek that was most likely mangled. Bicycle did not have any lights on it. Units canvassed the area and were unable to locate bicycle. Bicyclist was wearing a grey sweatshirt, black jeans and brown shoes. If the Bicyclist and his father find the bicycle later they will call NPD to canvas the area. Traffic Bureau update (Officer Gaudet): On March 1, 2022, I was able to speak with the injured bicyclist, Mr. Yonatan Gershon. Mr. Gershon stated he was traveling on Walnut Street (N) in the right shoulder lane towards the area of Hull Street and Kirkstall Road. Walnut Street, Hull Street, and Kirkstall Roads are all public ways maintained by the City of Newton. Mr. Gershon stated while operating his blue Trek gravel bicycle he continued on Walnut Street when a black vehicle (no further description) appeared out of no where on his left hand side and entered his travel lane. Mr. Gershon stated he did not have time to react and crashed into the rear passenger side of the vehicle. Mr. Gershon stated he does not recall anything between the time he made contact with the vehicle and the time he came to a rest on the roadway. Mr. Gershon stated the vehicle never made an attempt to stop and it left the area. Mr. Gershon stated after the crash he began walking towards the Newtonville Square area until his father picked him up and brought him to Newton Wellesley Hospital. Mr. Gershon stated he suffered injuries to the right side of his face, his right should, right hand, and was diagnosed with a concussion as a result of the crash. Mr. Gershon initially believed the vehicle entered Walnut Street from either Kirkstall Road. Kirkstall Road is on the Eastbound side of Walnut Street and if a vehicle exited the street onto Walnut Street it's driver side would be facing Mr. Gershon's direction of travel. Hull Street is on the Westbound side of Walnut Street. Hull Street supports one-way traffic and traffic can only enter Hull Street from Walnut Street. If the vehicle were to enter Mr. Gershon's travel lane as he described, the vehicle would have to travel Eastbound on Hull Street in violation of the one-way restriction to take a left onto Walnut Street and travel Northbound. Mr. Gershon stated it is possible the vehicle may have exited a driveway from a residence on the Westbound side of Walnut Street, taking a left turn to travel Northbound. After speaking with Mr. Gershon about his injuries and his attempt to avoid crashing into the vehicle, we surmised Mr. Gershon attempted to travel around the vehicle's left side prior to impact, with the right side of his person crashing into the vehicle. Mr. Gershon stated he was not wearing a helmet at the time of the crash and there were not lights or reflective equipment on his bicycle. Mr. Gershon stated he was wearing a gray and white hooded sweatshirt with jeans at the time of the crash. Mr. Gershon stated he returned to the scene after the crash and discovered his bicycle resting near the lawn of a residence on Walnut Street in the area of the crash on the Westbound side of the street. Mr. Gershon stated there was extensive damage to the front wheel and the frame of the bicycle was bent in half. I canvassed the area for black vehicles with fresh damage to their rear passenger side with a negative result. I also canavassed the area for cameras with a negative result. To be further investigated.

Operator of vehicle one James Cellucci stated that he was driving south bound on Lexington St near number 419 in his 2012 Jeep Grand Cherokee ( MA reg 9339AC ). Cellucci stated his speed was approximately 10 to 15 MPH. Cellucci stated that a group of approximately 5 children ran out from between vehicles that where stopped in traffic in the north bound lane of Lexington St. Cellucci stated that one of the children ran in front of his vehicle. Cellucci stated that he attempted to break but a crash was inevitable. Witness Molly Van Horn made the same statement as Cellucci about a group of children running out between stopped vehicles . The child that was struck by Cellucci's vehicle was Ben Shreve ( DOB 12/14/2010 ) . Shreve did not lose consciousness as a result of the crash. Shreve was transported to Boston Children's Hospital via EMS for treatment along with his parents. Lexington St is a public way in the City of Newton.

On Friday April 8, 2022 at approximately 1230 hours I was dispatched to 36 Glen Ave for a single car MVA into a stone wall. The sky was clear at this time of day but the roads were wet due to a previous rain storm. Witness stated MV1 was operating southbound on Glen Ave (public way) prior to 36 Glen Ave at approximately 40mph. Be advised the speed limit of Glen Ave is 25mph. Witness said MV1 crossed into the northbound lane, and operated through the driveway entrance and into the stone wall belonging to Glen Ave Association. Witness stated there were no other MVs on the road at this time and they were unsure if MV1 operator was on their cellphone while driving. Witness stated MV1 operator crawled out from the vehicle and there were no one else involved or in the car. MV1 sustained heavy and extensive front end damage, and was towed by Todys. MV1 operator stated they only had chest pains, and was not suffering from any head or neck injury at this time. Due to the nature of the incident and quick response by Newton Medics, I was unable to speak to MV1 operator, who was transported to Newton-Wellesley Hospital. I was able to speak to an employee of Glen Ave Association and advised them of the incident. The employee was the manager and they stated the damage to their stone wall was approximately $5,000 USD, and their telephone extension was x200. The employee was given the accident report number.

On location I observed the two involved vehicles parked curbside along the westbound side of Washington st directly across from the NWH entrance. V1; 2014 Honda/CR-V Util. color black. The traffic signals at the time were on flash red &yellow instead of normal red, green and yellow. The morning commute traffic on Washington St. is heavy at that time of the morning. V1 operator stated she was waved on by the vehicle operator in the turning lane on the westbound side. V1 proceeded to turn left/westbound onto Washington St. and collided into the driver's side of V2 travelling straight ahead westbound in the outside//32 lane of Washington St. ( westbound). V1 Operator stated she did not see V2. V2 confirmed V1 operator's account of the collision. V2 operator also confirmed the traffic lights were on flash red/yellow. Neither operator was injured as a result of the collision nor was there airbag deployment in either vehicle. Neither vehicle requried towing service.

On 4/21/22, I was dispatched to the intersection of Crafts St./California St. for a motor vehicle collision involving a single vehicle into a tree. Upon arrival, I observed MV#1 had crashed into a tree in front of 315 Craft St. The operator stated the "road was slippery" and he veered off the roadway, hitting the curb, before traveling 20-25 yards on the sidewalk, hitting two tree's. The operator of MV#1 reported she was traveling westbound on Commonwealth Ave. prior to colliding into the light pole, but states she does not remember the moments prior to the collision.The operator of MV#1 reported she was uninjured, but was transported to Newton-Wellesley Hospital as a precaution. Newton Fire personnel determined the damaged light poll was not a hazard while Eversource was notified regarding their damaged poll. A witness to the collision reported they observed MV#1 traveling westbound on Commonwealth Ave. at approximately 20-25 mph, before it veered off the roadway and into the light poll. After colliding into the light poll MV#1 crossed over the double yellow line into oncoming traffic and hit the curb on the eastbound side of Commonwealth Ave. Pictures of MV#1 and the Eversource light poll were taken on the N493 camera and submitted to the NPD IT bureau. A Request for an Immediate Threat License Suspension/Revocation was faxed over to the Massachusetts RMV on behalf of the Operator of MV#1 due to the circumstances of this collision.

Operator of MV#1 stated he was being chased by Operator #2 from Rt. 95 N onto Rt. 30 (Commonwealth Ave) .According to Operator #1 both vehicles exited 95N and proceeded eastbound on Commonwealth Ave. towards Auburn St. Operator of MV#2 stated MV#1 pulled in front of his vehicle, traveled a few yards before abruptly attempting to turn left into the westbound lane of Commonwealth Ave. This abrupt turn caused MV#2 turn collide into rear side of MV#1. Damage to both vehicles consistent with operator of MV#2's statement. Operator of MV#1 stated he was "trying to pick up his mother" from work, so he attempted to turn left into the westbound lane of Commonwealth Ave. Both operators were transported to Newton-Wellesley Hospital for evaluations. Tody's towed both vehicles from the scene. Witness to collision confirms statement made by operator #2, indicating operator of MV#1's attempted left turn caused collision. Further statement by witness contradicts operator #1's statements of being chased as she witnessed MV#1 in front of MV#2. Additional investigation by officers led operator of MV#1 being cited for Operating Under Influence of Liquor (Mgl. Ch. 90 Sec. 24J) and Negligent Operation of a Motor Vehicle (Mgl. Ch. 90. Sec. 24E).

The operator of MV#1 stated he was traveling northbound on Highland St. when he became distracted attempting to locate the street sign to input into his GPS. The driver's lack of attention to the roadway resulted in his vehicle rear ending MV#2 (unoccupied) which was parked facing northbound in front of 60 Highland St. MV#1 then continued into the parking lot of 60 Highland St. MV#1 sustained heavy front end damage including deployment of both front and side airbags. The operator of MV#1 was later transported to the hospital and his vehicle towed from the scene. MV#2 which sustained a flat rear tire and apparent damage to its axle was also towed from the scene, as it was obstructing the flow of traffic. Attempts to make contact with the owner of MV#2, were unsuccesfull.

-Ped#1 was crossing Auburn Street southbound when she was struck by MV#1. MV#1 left the scene of the collision and did not leave any contact information. -Ped#1 stated she was crossing Auburn Street, after signaling the crosswalk. She stated she was in the crosswalk when MV#1 kept driving. Ped#1 stated she jumped back wards to avoid being struck,however, she clipped on the left ankle. She identified the driver as an old man. -I next spoke to Wit#1 and her statements regarding the collision coincided with Ped#1 Wit one went on to describe MV#1 as a dark gray Prius; that it pulled into the MBTA parking lot after PED#1 went to get her residency manager; and lastly she observed MV#1 leave the scene of the collision. Wit#1 stated she did not get the registration for MV#1. -Ped#1 was transported to Newton-Wellesley Hospital for a minor injury to her left foot where she was struck by MV#1 while jumping out of the way. There was no cameras nor other witnesses found at this time.

Narrative:
On Tuesday, June 28, 2022, while assigned to unit N496, I responded to the area of Langley Road and Hamlet Street, Newton for a report of a roll over crash with 3 injured parties. The weather at the time of this crash was clear and sunny. The road surface in the area of the crash was dry. Langley Road and Hamlet Street are both public ways maintained by the City of Newton. I approached the area of the crash coming from Boylston Street and when I arrived my attention was drawn to two locations on Langley Road. I observed a female party laying prone on the sidewalk area of Langley Road (E) at the Hamlet Street intersection. This female party, later identified as Ms. Melinda Conroy, was being attended to by passerby's in the area. My attention was then drawn to an overturned vehicle resting half in the roadway/half on the sidewalk (W) in front of 323 Langley Road. As I began to run over to the vehicle, I observed a woman standing next to the vehicle screaming frantically. This party identified herself as the driver of the lone vehicle involved, Ms. Natalya Kabanovsky (S65512051). There were parties at the back of the vehicle yelling to me that there was a woman trapped under the vehicle and that they were holding the vehicle up to get her out. I ran over to the rear of the vehicle and observed a female party, later identified as Ms. Annette Sooper, lying underneath the vehicle in the roadway. There was a large pool of blood on the roadway as the result of a laceration to what appeared to be her head/face area. Due to the unknown stability of the vehicle resting on its roof, and due to the passerby's stating they were holding the vehicle up so it did not roll over onto Ms. Sooper, I reached under the vehicle from the rear, grabbed Ms. Sooper's person, and dragged her away from the vehicle to a safe location on the sidewalk. At this time, I observed a large laceration to Ms. Sooper's head and she stated she had suffered a leg injury. Newton Fire and Medics along with additional Police units arrived on scene and took over rendering aid to the victims. I then started to conduct my investigation into the crash. I spoke with the operator of the vehicle, Ms. Kabanovsky. Ms. Kabanovsky stated she was operating her 2005 Lexus ES330 (MA: 293TAB) on Hamlet Street (W) towards Langley Road. Ms. Kabanovsky stated as she approached the intersection, a female party (Ms. Conroy) ahead of her was running across Langley Road (S) at Hamlet Street. Ms. Kabanovsky stated she attempted to hit her brakes, but instead hit her gas pedal. Ms. Kabanovsky's vehicle crashed into Ms. Conroy and then she panicked. Ms. Kabanovsky stated her car then accelerated across the street and crashed. Ms. Kabanovsky stated she was not wearing a seatbelt at the time of the crash and extricated herself from her vehicle after it rolled over onto its roof. I requested Ms. Kabonovsky's license while speaking to her, but she was too upset to located it. I queried Ms. Kabanovsky's license and registration status through Newton Dispatch. Ms. Kabanovsky's license stated is expired as of 12/18/2016. Ms. Kabanovsky registration is active. I observed substantial damage to the entire front end, driver, and passenger side of Ms. Kabanovsky vehicle. Ms. Kabanovsky was evaluated by Newton Medics and complained of right arm pain. Ms. Kabanovsky was also visibly distraught and expressed concern over the two pedestrians involved in the crash. Ms. Kabanovsky was transported to Brigham and Women's Hospital for treatment. I briefly spoke with Ms. Melinda Conroy as she was being attended to by Newton Medics and Fire. Ms. Conroy stated she was running on Langley Road (S) and observed MV1 traveling on Hamlet Street (W) towards Langley Road. Ms. Conroy stated she believed MV1 was slowing down and coming to a stop. Ms. Conroy stated she waved at the vehicle and as she crossed the intersection, MV1 accelerated and crashed into her left side. Ms. Conroy stated she flipped over the vehicle and landed on the roadway. Ms. Conroy was conscious and alert when speaking to Newton Medics and was transported to Beth Israel Deaconess Hospital in Boston. I later spoke with Ms. Conroy after her release from the hospital. Ms. Conroy stated she suffered some bumps and bruises had no major injuries. Officer Pohlman spoke with Ms. Sooper prior to her transport to Beth Israel Deaconess Hospital in Boston by Newton Medics. Ms. Sooper stated she was walking on the sidewalk in front of 323 Langley Road after returning from Newton Centre. Ms. Sooper stated as she was walking, she observed MV1 hit Ms. Conroy and then travel across the street in front of her into a tree. Ms. Sooper stated she does not remember what happened after that. I spoke with Ms. Sooper's son at NPD HQ and was advised Ms. Sooper was admitted to the hospital and suffered a fracture of her left hip and required 6 staples to close a laceration on her head. Tody's Towing responded to the scene and removed MV1 from the roadway. Traffic Officer Wilson took pictures of the scene and submitted them to the IT Bureau. After speaking with all parties involved, Ms. Kabanovsky will be cited with Massachusetts Uniform Citation 494688AB in hand at her residence for Chapter 90, Section 10 (Unlicensed Operation), Chapter 90, Section 24 (Operate To Endanger), and Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Ms Kabanovsky was also cited with Massachusetts Uniform Citation 494800AB in hand at her residence for Chapter 90, Section 13A (Fail To Wear A Seat Belt). A Request for Immediate Threat License Suspension/Revocation was completed and submitted to the Registry of Motor Vehicles with supporting documentation via e-mail, DCUImmediateThreat@massmail.state.ma.us.
