On 1/19/19 I responded to 25 Normandy Rd for a report of a past hit and run. I spoke with the Operator of MV#1 who stated the accident had taken place in front of Tom's Pizza, 423 Lexington St, at approximately 1830 hrs. Operator of MV#1 stated she parked her vehicle in a parking spot directly in front of the restaurant and went inside. When she returned approximately 5 minutes later she realized her vehicle had been rear ended. Two witnesses provided her with information about the accident and the plate number. (See witness narratives.) MV#1 suffered moderate damage to the rear and was driven from the scene. Waltham PD responded to the address listed for MV#2 and were not able to locate the vehicle. At approximately 2000 hrs, I was contacted at the police station by the owner of MV#2's wife. After further investigation (see incident report #19002425) it was determined that the operator of MV#2 at the time of the accident was the vehicle owner's brother. I spoke with Operator of MV#2 on the phone and he stated that the road was icy and slippery due to snow and he crashed into the back of MV#1. He stated he was nervous and thought he had only hit the sidewalk curb. Operator of MV#2 was issued citation T1440626 by mail for 90/24C Leaving the Scene of Property Damage, 90/10A Unlicensed Operation and 90/24 Use of MV Without Authority. He stated the vehicle suffered minor damage to the front.

MV1 (MA Reg: 63EP17, a 2000 Acura 32TL; operator: Carlos Standford) was traveling NORTH on Cherry St when a vehicle swirved into his lane head-on. In order to avoid MV3 he swirved into the other lane and side-swiped MV2. MV2 (MA Reg: 9JX388, a 2019 Subaru Outback; operator: James Cant) was traveling SOUTH on Cherry St when he had come to a complete stop, put his left turn signal on to show him turning left into his driveway at 176 Cherry St. He was waiting to make his turn until MV1 had driven past so it was safe to do so. As he was at a complete stop, MV1 collided with his rear driver side bumper in an effort to avoid a head on collision with MV3. MV3 (MA Reg: 93YE85; a 1997 Toyota Camry; operator Matthew Kuniholm) was traveling SOUTH on Cherry St when he had seen MV2 at a complete stop with a turn signal. He didn't give himself enough time to stop, began to slide as he was braking and went over the marked lanes into oncoming traffic in an effort to not collide with MV2. MV3 operator, Matthew Kuniholm, was issued in hand MA Uniform Citation T1440470 for Marked Lane Violation 89/9a. All vehicles were still in driveable condition. MV1 had damage to the front driveside wheel fender and driver door. MV2 had slight damage to rear driver side bumper. MV3 did not have damage to his vehicle. Weather was icy and there was an active snow fall during the time of the accident, which was 1815hrs.

On the above date/time I responded to the area of 82 Charlesbank Rd for a report of a DMV. Upon arrival on scene I met with the operator of vehicle one Mr. Frank. He stated he was traveling south on St. James and turning right onto Charlesbank Rd. He further stated as he turned right the vehicle was uncontrollable and continued straight due to the decline and icy roadway and struck the island curb on Charlesbank Rd. Due to the heavy damage to the left side and front the vehicle was towed by Tody's towing of 1354 Washington Street 617-527-0013. Tody's towing arrived on scene and towed the vehicle to their tow lot. I transported Mr. Frank to his residence.

On the above date/time I responded to 503 Watertown Street for report of a mva hit/run with property damage. Upon arrival I met with the owner of 503 Watertown Street Ms. Antonietta Tempesta she stated through he daughter in law Ms. Tiz Acuri at approx. 0830 hours she noticed fresh tire impressions on the roadway leading onto the sidewalk and into her fence that was damaged. She also stated the MA registration plate # 5DT664 was on the fence. I ran the the plate which came back to a 2003 Ford pickup color white to Kyle Butler of 84 Auburndale Ave Newton MA. Officer Boyle in unit 493 went to the residence and spoke with Kyle's uncle to call NPD. I advised Ms. Acuri of the process. Mr. Butler called NPD I spoke with him on the phone. He stated he was driving west on Watertown Street and due to the icy roadway his truck slid to the right onto the sidewalk and into what he thought was a snow bank. I gave all parties each others information and advised them. I gave the uncle Kyles reg plate.

On 1/21/19 I was working N498 when I responded to 17 Charles River Ter for a report of a Cab that had struck a wall. Upon arrival I met with Pema Jigmey and Miriam Jost. Jost stated that Pema who was driving for Metro Cab (or Photo Cab) picked her up at the airport and brought her home. Once at her home 17 Charles River Ter, she asked him to back into the driveway due to the snow and ice conditions. While backing up in the driveway of 17 Charles River Ter the vehicle struck the residencet causing damage. Pema stated the same thing but believed Jost was half to blame due to the fact she asked him to be dropped in the driveway. I informed Pema that he drove the vehicle into the house and so was at fault. The house only appeared to suffer minor cosmetic damages but could not be accurately examined. Pema and Jost exchanged information in the hopes of settling the damages without insurance. Both parties were satisfied and provided a report number. Both parties stated they were uninjured and refused medical attention.

On the above date/time I responded to the area 2000 Washington Street Newton -Wellesley - Hospital for a two motor vehicle crash on the 3rd floor of the parking garage. Upon arrival I met with the operator of vehicle one, she stated she was pulling into a parking spot and applied the brakes but due to the ice on the roadway and slope of the roadway her vehicle slid into the rear of vehicle two which was parked. Vehicle two was unoccupied, and dispatch tried to get in contact with the owner to advise them. I also left a message with NWH in case the owner comes back in. No injuries and I advised of the process. Later in the day the owner vehicle two came to NPD, I gave him vehicle one inforamtion and advised him of the process.

On 01/31/19 at 10:39 hours, I responded to the parking lot of the Jewish Community Center located at 333 Nahanton Street for a motor vehicle hit and run accident. The parking lot is accessible to the public and designed for vehicles to travel to and from the JCC. Owner of M/V 1 states she was parked on the side of the handicapped parking stall thinking it was ok to park there. She then ran in to the JCC for approximately one minute and when she returned she observed her driver's side front door was struck. I observed significant damage to the door frame and the door would not close. The owner called for a private tow to remove the vehicle. The staff at the JCC stated that a Martha Bixby gave her number and stated that she observed a black colored car with MA reg. # 934HH4 strike M/V 1. I attempted to call the number and left a message. The listing for that plate comes back to a 2010 Lexus ES350 color gray. I was then showed the video surveillance from the JCC's cameras and it shows an older white female exiting the lobby wearing a winter hat and carrying a bag with purple flower designs on it. She then walks through the lot and enters a gray Lexus ES350. The Lexus then starts backing out of the spot. The Lexus then continues backwards in to the other side of the parking stalls and strikes M/V 1 on the driver's side front door. The Lexus then pulls forward and leaves the scene without stopping or providing her information. As the Lexus is leaving, you can see there is a dent to the driver's side rear bumper. The video is not clear enough to see the plate on the Lexus or clearly see the operator's face. After, I located a number for the owner of the Lexus M/V 2 (Tania Lefman). I spoke to Lefman on the phone and she stated that she was backing out of the JCC parking lot at that time and felt a tiny bump to her rear end. She says that she could not stop in time due to the icy parking lot and then left the area thinking that nothing was wrong, it was only minor. I advised her there was significant damage to the other car. She was issued and mailed MA Uniform Citation T1441054 for Ch. 90/24/C Leaving the scene of property damage.

MV#1 Stated while traveling westbound down Commonwealth ave he struck a patch of ice. The ice caused his vehicle to rotate 360 degrees and proceeded to slide on the roadside shoulder striking a verizon poll ( 102/285). MV#1 traveled approximately 12ft from the road before striking the poll. MV#1 sustained heavy front end damage. MV#1 had all airbags deployed and had a small fire under the front end. I observed multiple other vehicles passing having difficulties and vehicles sliding due to significant ice conditions. Driver of MV#1 had injuries to his left arm, possible broken forearm/wrist. Passenger of MV#1 stated she had chest pains. Medics transported both parties to Newton Wellesley Hospital for further evaluation. Tody's Towed the vehicle due to heavy front end damage.

Operator of MV # 1 states that she took a right turn off of Parker Street on to Kendall Road when she lost control due to snow and ice on the right side of the road. The vehicle then veered off to the right over the curb and struck a United States Post Office Relay Mailbox knocking it completely over. The mailbox was located in front of 7 Kendall Road. No injuries reported by the operator and the vehicle sustained minor front end damage to the right side. I did observe an accumulation of snow and ice on the right side of Kendall Road that came out approximately one foot from the curb. Newton Center Post Office was notified regarding the mailbox. There was no mail in the box at the time.

On Thursday, February 21, 2019, while assigned to Traffic unit N526, I responded to the area of Ionia Street, Newton for a report of a motor vehicle accident involving a City of Newton owned vehicle. Ionia Street is a public way maintained by the City of Newton. The weather at the time of the accident was overcast. The road surface was icy and snow covered. The operator of MV1, Patrick Carresi (MA: S73958042), stated he was operating a City of Newton owned 2016 Ford F350 plow truck (MA MVN: M94756) turning left onto Ionia Street from Auburndale Av. Carresi stated as he took the turn, the rear passenger corner of the truck made contact with the drivers side front panel of MV2 which was parked on the side of the road facing southbound. I observed minor damage to the rear of MV1. MV2 received heavy front end damage with the entire front panel torn off. Carresi reported no injuries. Carresi works for the Utilities Department with the DPW. The operator of MV2, Donna Canavan (MA: S57637058), stated she was parked inside her 2005 Honda CRV (MA: 8ZEE40) facing southbound on Ionia Street. Canavan stated as the plow truck took the turn onto Ionia Street from Auburndale Av its rear corner of the truck struck the front panel of her parked vehicle ripping the panel off. I observed significant damage to the front panel area of MV2. Canavan reported no injuries and was able to drive her vehicle from the scene. Pictures were taken of both vehicles and submitted to the IT Bureau.

Operator of MV#1 stated he was travelling westbound on Commonwealth Ave with his dog in the front seat when he slid on ice and off the roadway. The roadway was wet from previous snow fall but was very well plowed and sanded. MV#1 hit a curb, drove through a snowbank, hit a tree with the left front of his vehicle and then hit a pole with the right front side of his vehicle. The car then came to a rest in the median. The operator appeared confused by the accident and did not realize the extent of the damage to his car.He asked what he had hit. He continuously asked if it was driveable even though the entire front end was totaled. He was able to provide me with his license and all pertinent information but spoke in a slow manner. There was no odor of alcohol. Operator declined medical attention. When asked if he was on any medication he stated he was on heart medication. We were able to contact the operator's son who was transported to the scene and stated the operator had previously suffered a stroke. David Donahue arrived on scene towed the vehicle to Tody's. A licensed driver picked up the operator. A search of the immediate area yielded negative results for ice in the roadway. A request for Immediate Threat License Suspension/Revocation was filed.

Operator of Vehicle #1 stated that he was traveling North on Centre Street. He passed through the intersection of Cabot and Centre street where he proceeded downhill on an icy road. Operator stated that he "fishtailed" and lost control of his vehicle before leaving the roadway and coming to rest in the shrubs and trees in the front yard of 700 Centre Street. At the time of this incident there is a reported water main break at the intersection of Centre street and Sargent street with City Water Crews addressing the issue. I observed a large collection of ice in the intersection and in the northbound lane of Centre street. No injuries were reported. Tody's was notified and removed the truck from the bushes. Operator #1 drove the vehicle away. I notified the home owner, Mr. Watkins of the damage. He estimated the damage to the landscaping to be approximately $2000.

Operator #1 stated as he was rounding a slight bend in the Rd. His vehicle continued to slide to the left and into oncoming Eastbound traffic. Vehicle #1 Then collided with vehicle #2. On the above date and time I responded to 717 Centre St for a report of a crash involving two vehicles. The weather was snowy / rainy/ sleeting, and the roadway was a mixture of ice/ snow. Upon arrival on scene I spoke with the operator of V1 Ms. Howe, she stated she was traveling south on Centre St when V2 stopped abruptly. Ms. Howes stated she applied the brakes and her vehicle would nt stop due to the icy roadway and she had contact with V2. I then spoke with the operator of V2, Ms. Nunnally stated that a vehicle had abruptly pulled out of the driveway of 717 Centre St. She stated she applied her brakes immediately to avoid contact and pulled to the right. She stated when she stopped she felt rear impact from V2. Both parties had no injuries at this time. V2 was towed by Todys towing to 1354 Washington Street Newton. Ms. Howe removed valuables from the vehicle. At this time it appears the icy roadway was a contributing factor of the crash.

On 12/30/19 I responded to a single vehicle crash on Dedham St. outside of #268. The operator stated she was traveling due North on Dedham St at 25mph when she lost control of her vehicle due to icy conditions. Tracing back the vehicles tires marks in the slush, the car appeared to cross the double yellow lines near Brush Hill Rd and then back to the correct lane sideways before striking the curb, then a city tree in front of #268 Dedham St. There were multiple air bag deployments in the vehicle. Ambulance #1 arrived on scene and transported the operator due to a complaint of right chest pain from the seatbelt. N523 Traffic Officer Wilson arrived on scene and took pictures of the damage to the city tree. On 12/05/2020 at approximately 22:00 hours I was called in to respond to a single vehicle crash with serious injuries in the area of 291 Nahanton St. Nahanton St is a pubic way in the City of Newton. Upon my arrival at the crash scene with Sgt Devine I was briefed by Officers Daly and Anderson, ( see Ofc Daly's report for further information CAD # 20054728 in regard to changes take out against the operator of the truck ) I made the following observations of the crash scene, Nahanton St is a public way in the City of Newton that is a two way roadway divided by an unprotected merdium ( double yellow line ) that runs east to west. The roadway surface was wet due snow and ice. The roadway was plowed and the snow was pushed to the shoulder, althought wet the roadway was icy. There was approximately 2 inches of snow on the shoulders of the roadway. The road was light with overhead head street lights. The posted speed limit is 25 MPH. I observed a 2013 Chevrolet Silverado pick up truck that had major front end damage. The Chevrolet was at an angle with it's front end facing west bound and resting on the grass shoulder and into a wooded area. The Chevrolet rear was sticking out into the shoulder of the east bound lane of Nahanton St. The passenger side front and rear doors of the Chevrolet had been removed by the fire department to extricate the driver and passenger. The driver and passenger side air bags were deployed . The front passenger side windshield of the Chevrolet was completely missing and the driver's side had completely shattered. The front tires, hood, and engine had been compacted on it's self. The front and rear driver's side doors had been mechanically open by the fire department. The speedometer needle had stopped at 54 MPH. The Chevrolet left the roadway and left tire marks in the snow in the eastbound bike lane of Nahanton St. At a 106.4 feet from the beginning of the tire marks the first point of impact was with two tree stumps that were 3.5 feet from the roadway, four feet tall and 3.5 feet in diameter. The tire marks continue in the east bound bike lane and exit the roadway and onto a dirt shoulder at 136.1 feet from the beginning of the tire marks. The truck continued to the right of a utility pole and then it's passenger side made contact with a tree at 170.8 feet from the begin of the tire marks. The tree was 12 feet from the bike lane approximately 5 feet in diameter. The impact with the tree caused the truck to turn so that the rear was facing north east on the dirt shoulder and bike lane. The final point of rest of the rear passenger side tire was 200 feet from the beginning of the tire marks. The truck's rear diver side tire final point of rest was 209 feet from the beginning of the tire marks. The front driver's side tire was compacted into the trucks engine and the passenger side front tire was located behind the tree approximately 20 feet from the roadway. Sgt Devine took photos of the crash area and of the truck and the disk was download by the NPD's IT Bureau. The truck was towed from the crash scene by Todys's Towing to their lot for storage, I fill out a towed motor vehicle form. There were no reported witness who observed the crash. The crash was reported by an unknown passerby. On 12/07/2020 Ofc Gaudet faxed a request for preservation of medical records, blood and urine samples of Schneider to Beth Israel Deconess Medical Center.

On 12/08/20 at 08:42 hours, I responded to 26 Cummings Road for a city motor vehicle accident involving a parked unoccupied motor vehicle. Cummings Road is a public way maintained by the City of Newton. The weather at the time was approximately 30 degrees and very cloudy. On arrival, I spoke to the operator of M/V # 1 Alphonse Secinaro MA DL# S88518110 who works for the City of Newton Dept. of Public Works Yard out of 74 Elliot Street. Mr. Secinaro stated at the time he was sanding or cleaning up icy spots on the roadways. He was operating a 2012 International 7 Series color orange (MA Official Reg# M87605) with a plow attached to it. While driving northbound on Cummings Road between number 26 and 34, Secinaro attempted to maneuver his large plow truck between two parked motor vehicles on opposite sides of the roadway. While trying to do so, he struck parked M/V # 2 (14' Honda Pilot MA # 7RT637) on the driver's side rear parking light area. This caused the brake light cover to break and a small dent to the vehicle to the left of that brake cover. I observed M/V # 1 to be parked on the right side of 26 Cummings Road partially on the grass where there is no sidewalk. The other vehicle was parked a little more southbound on the opposite side of the roadway which made it very difficult for the large plow truck to navigate through. I observed no parking restrictions for both sides of the street. Digital photos were taken of the scene and submitted to the IT Bureau.

MV1, Mr. Marchand was driving Southbound on Highland St when a vehicle stopped abruptly in front of him to allow a vehicle to enter the travel lane exiting out of Ascenta Terrace. When the vehicle stopped abruptly in front of him, MV1 slammed on the brakes and did not hit the car in front of him, however he was rear-ended by a vehicle behind him, MV2. MV2, Mr. Calderon, was traveling Southbound on Highland St behind MV1 when he observed MV1 brake. He then immediately stepped on his brake and was not able to maneuver the vehicle in order to avoid the collision due to icey, slippery conditions on the road. MV2 sustained heavy front end damage. MV1 sustained minor damage to the rear he vehicle. No injuries were reported on scene. MV2 had to be towed by Tody's as it was disabled.

On 02/25/2021, while assigned to N494, I, Officer Conary responded to 41 Algonquin Road for a motor vehicle that struck a parked motor vehicle. The weather was cold outside and the ground in the area had iced over. I spoke with Operator of MV1 who stated he was driving Northbound on Algonquin Road when he lost control of his vehicle due to the ice on the road, and hit MV2 and then hit a snow bank. I asked he wanted medical attention and he declined. There was damage to MV2 left rear bumper and tire. MV1 had damage to the front bumper. MV2 remained parked on scene, and MV1 was able to be driven. Owner of MV2 and Operator of MV1 exchanged necessary paperwork. City was notified of the ice and also debris in the road.

On Tuesday, March 2, 2021 while assigned to Traffic unit N525, I responded to the area of Colonial Avenue with Newton unit N491 for a report of a hit and run that just occurred on Linwood Avenue. The reporting party stated a U-Haul (AZ reg: AE56270) was observed hitting the driver side door mirror of a pick up truck parked in front of 232 Linwood Avenue, Newton and leaving the area. Linwood Avenue and Colonial Avenue are both public ways maintained by the City of Newton. Officer Lazarakis located this vehicle in front of 14 Colonial Avenue. The operator of the U-haul was identified as Mr. Marco Mata (S16461888). Mr. Mata stated he was unaware the vehicle he was operating made contact with the other vehicle. Mr. Mata stated the vehicle is large and he believed the noise he heard was ice falling off the vehicle. I observed minor damage to the passenger side door mirror of MV1. Mr. Mata was advised and cited with Massachusetts Uniform Citation 920282AA for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). No injuries reported by Mr. Mata or his passenger Mr. Donovan Michel (SA3950535) The registered owner of MV2, Mr. Jeffrey Boudreau, was advised. I observed moderate damage to the driver side door mirror of MV2.

Operator of MV 1 was traveling northbound on Cherry Street when she struck a parked vehicle MV2 and then struck an additional parked vehicle MV3. Both MV2 and MV3 were unoccupied.
