On January 1, 2019 at approximately 02:18 hours I, Officer Guarino, responded to 78 Vine St. for a hit and run. I spoke to the operator of MV1, who told me that he was driving Eastbound on Vine St, he turned on his left directional and began to pull into his driveway at 78 Vine St. The operator of MV 2 attempted to pass MV 1 on the left hand side crashing into MV1. MV2 put the vehicle in reverse and left the scene. The operator of MV1 said that the front bumper of MV2 had fallen off in the collision with MA License plate 4KZ917 still attached. The owner of that vehicle lives in Boston, so Boston Police were notified. I attempted to make contact with the owner of MV 2 with negative results. Update January 1, 2019 16:15 hours: I was able to contact the owner of MV2, who admitted that she was the operator of the vehicle and that she was in the vehicle alone. The operator of MV2 said that the operator of MV1 was driving very slow and stopped in the right lane so she attempted to pass the vehicle. MV1 began turning and MV2 struck MV1. The operator of MV 1 said that she was nervous and left the scene because her phone died and attempted to contact the Newton Police in the morning. I am mailing citation T1441275 to Cecelia Spangenberg for MGL90/24C, leaving the scene of property damage. Photographs of the bumper have been submitted to IT to be attached to this report. The license plate was logged into evidence and placed into property and evidence locket #8.

On 1/19/19 I responded to 25 Normandy Rd for a report of a past hit and run. I spoke with the Operator of MV#1 who stated the accident had taken place in front of Tom's Pizza, 423 Lexington St, at approximately 1830 hrs. Operator of MV#1 stated she parked her vehicle in a parking spot directly in front of the restaurant and went inside. When she returned approximately 5 minutes later she realized her vehicle had been rear ended. Two witnesses provided her with information about the accident and the plate number. (See witness narratives.) MV#1 suffered moderate damage to the rear and was driven from the scene. Waltham PD responded to the address listed for MV#2 and were not able to locate the vehicle. At approximately 2000 hrs, I was contacted at the police station by the owner of MV#2's wife. After further investigation (see incident report #19002425) it was determined that the operator of MV#2 at the time of the accident was the vehicle owner's brother. I spoke with Operator of MV#2 on the phone and he stated that the road was icy and slippery due to snow and he crashed into the back of MV#1. He stated he was nervous and thought he had only hit the sidewalk curb. Operator of MV#2 was issued citation T1440626 by mail for 90/24C Leaving the Scene of Property Damage, 90/10A Unlicensed Operation and 90/24 Use of MV Without Authority. He stated the vehicle suffered minor damage to the front.

On the above date/time I responded to 503 Watertown Street for report of a mva hit/run with property damage. Upon arrival I met with the owner of 503 Watertown Street Ms. Antonietta Tempesta she stated through he daughter in law Ms. Tiz Acuri at approx. 0830 hours she noticed fresh tire impressions on the roadway leading onto the sidewalk and into her fence that was damaged. She also stated the MA registration plate # 5DT664 was on the fence. I ran the the plate which came back to a 2003 Ford pickup color white to Kyle Butler of 84 Auburndale Ave Newton MA. Officer Boyle in unit 493 went to the residence and spoke with Kyle's uncle to call NPD. I advised Ms. Acuri of the process. Mr. Butler called NPD I spoke with him on the phone. He stated he was driving west on Watertown Street and due to the icy roadway his truck slid to the right onto the sidewalk and into what he thought was a snow bank. I gave all parties each others information and advised them. I gave the uncle Kyles reg plate.

On the above date/time I responded to 647 Washington Street for a report of a two vehicle crash. Upon arrival I met with the operator of vehicle two Mr. Weiner, he stated he was stopped in the Whole foods parking lot when vehicle one backed up into his vehicle. They exchanged information but the operator of vehicle one Ms. Adetola left prior to my arrival. I obtained her license information through Mr. Weiner and when I ran the reg plate on vehicle one it came back to Watertown Ford. I advised Mr. Weiner of the process and I will contact Watertown Ford for more contact information on Ms. Adetola to obtain a statement. Update:Jan. 21, 2019 On Jan. 21, 2019 I contacted Watertown Ford advised them of the crash and obtained a phone number for the operator of vehicle one. I spoke to her on the phone she stated the parking lot roadway was covered in snow/ice, she was backing out of the parking spot and vehicle two continued traveling and struck her vehicle. I advised her of the process.

Leary was crossing Needham St southbound at the corner of the property between Stop and Shop and Jiffy Lube. She states that she was 1/4 of the way across the street when she was struck by MV1. The left side of her body(arm, hip) struck the right side of the MV. She fell to the ground. MV1 states that he was pulling out of the Stop and Shop driveway to turn right onto Needham St. He saw the pedestrian next to the snow bank on the right side of the driveway as he approached the end of the driveway. When he got to the end he did not see her anymore and assumed she had continued walking west on Needham St. He then turned right and felt the collision and then he saw her. He immediately pulled into the Jiffy Lube lot to check on her. I took pictures of the scene and the MV. They were TOT IT. After further investigation, on 2/7 the operator was cited for 90/14 failure to stop for pedestrian. I spoke with Kenneth via phone and advised him of the update. The citation was mailed to him. I am charging for 90/14 failure to stop for a pedestrian. On Wednesday February 6, the operator information of this report was ammended to show the correct operator information.

On 01/31/19 at 10:39 hours, I responded to the parking lot of the Jewish Community Center located at 333 Nahanton Street for a motor vehicle hit and run accident. The parking lot is accessible to the public and designed for vehicles to travel to and from the JCC. Owner of M/V 1 states she was parked on the side of the handicapped parking stall thinking it was ok to park there. She then ran in to the JCC for approximately one minute and when she returned she observed her driver's side front door was struck. I observed significant damage to the door frame and the door would not close. The owner called for a private tow to remove the vehicle. The staff at the JCC stated that a Martha Bixby gave her number and stated that she observed a black colored car with MA reg. # 934HH4 strike M/V 1. I attempted to call the number and left a message. The listing for that plate comes back to a 2010 Lexus ES350 color gray. I was then showed the video surveillance from the JCC's cameras and it shows an older white female exiting the lobby wearing a winter hat and carrying a bag with purple flower designs on it. She then walks through the lot and enters a gray Lexus ES350. The Lexus then starts backing out of the spot. The Lexus then continues backwards in to the other side of the parking stalls and strikes M/V 1 on the driver's side front door. The Lexus then pulls forward and leaves the scene without stopping or providing her information. As the Lexus is leaving, you can see there is a dent to the driver's side rear bumper. The video is not clear enough to see the plate on the Lexus or clearly see the operator's face. After, I located a number for the owner of the Lexus M/V 2 (Tania Lefman). I spoke to Lefman on the phone and she stated that she was backing out of the JCC parking lot at that time and felt a tiny bump to her rear end. She says that she could not stop in time due to the icy parking lot and then left the area thinking that nothing was wrong, it was only minor. I advised her there was significant damage to the other car. She was issued and mailed MA Uniform Citation T1441054 for Ch. 90/24/C Leaving the scene of property damage.

OWNER STATES HIS VEHICLE WAS PARKED ALL DAY BECAUSE HE IS DOING WORK AT THAT LOCATION. COWORKER GOT THE PLATE OF THE VEHICLE THAT HIT HIS VEHICLE. OWNER REPORTS NO INJURY AND WAS ADVISED O CONTACT HIS INSURANCE COMPANY. ATTEMPTS TO MAKE CONTACT WITH THE OWNER OF VEHICLE #2 WAS UNSUCCESSFUL AT THIS TIME. FOUND A SECOND NUMBER ( 860-301-1336 ) FOR THE METROWEST PACKAGING COMPANY AND LEFT ANOTHER VOICE MAIL.WILL TRY TO MAKE CONTACT AGAIN BEFORE PURSUING OTHER OPTIONS. Traffic Bureau Update (Officer Gaudet): On Monday, February 4, 2019 I made contact with the President of Metrowest Packaging, Brian Hersh. Hersh stated his company does fulfillment deliveries for Amazon. Hersh was advised that a vehicle registered to his company was allegedly involved in a hit and run accident. Hersh was provided information on the vehicle involved. Hersh stated an employee named Alexis was operating MA CON:T65665 on the date of the accident and provided a phone number for this party. On Tuesday, February 5, 2019 I spoke with Alexis Laporte-Soto (S84358654). Laporte-Soto stated he was operating a 2017 Mercedes Sprint van (MA CON:T65665) in Newton for Metrowest Packaging on the date of the accident. I explained to Laporte-Soto the vehicle he was operating was alleged to have been involved in a hit and run accident on Highland Street in Newton. Highland Street is a public way maintained by the City of Newton. Laporte-Soto stated the vehicle he was operating hit the driver door mirror of a parked card on Highland Street. Laporte-Soto reported the accident as mirror (MV2 passenger side) to mirror (MV1 driver side). Laporte-Soto stated after his passenger side door mirror made contact with MV1's driver side door mirror, he stopped his vehicle 15 feet down the road. Laporte-Soto stated he walked up to the car he hit and saw the mirror was bent back but there was no damage. Laporte-Soto stated he waited near the car for 10 minutes and the owner never showed up. Laporte-Soto also stated a "random guy" took a picture of his license plate. Laporte-Soto stated he said "hi" to this party but the party ignored him and left the area. I asked Laporte-Soto why he did not call Newton Police to assist him in finding the owner of the vehicle he hit. Laporte-Soto stated he didn't know. I also advised Laporte-Soto that the name of the owner of the vehicle he hit was printed on the driver side door next. The owner's phone number and email address were also printed on the driver door. Laporte-Soto stated he didn't see that. I spoke with the owner of MV1, Jason Correria. Correria stated a co-worker observed the entire accident and observed MV2's passenger side mirror hit MV1's driver side mirror. Correria stated his co-worker then observed MV2 travel down Highland Street with out stopping and pull into the Santander parking lot located at 1314 Washington Street. Correria stated his co-worker saw the operator of MV2 exit his vehicle, check his passenger side door mirror for damage, and leave the area. Laporte-Soto will be mailled Massachusetts Uniform Citation T1443121 for Chapter 90, Section 24 (Leaving the scene of property damage).

The operator of MV1 stated she was parked SB in front of 175 Temple St. from approximately 1000 hrs to 1430 hrs, and she came out to a note left on her vehicle. The note stated "I saw someone back into your car, I got their plate, MA Reg TC58LC, Black Suburban". and there was minor damage to the drivers side doors. MA Reg TC58LC came back to a HUTTER, Carol of 67 Randlett Pk, Newton. I responded to that address and attempted to raise HUTTER with no answer at the door. I then called the phone number listed from a past incident with the Newton Police and HUTTER answered the phone. She stated she was at the Temple attempting to parallel park when she thought she may have hit another vehicle. She stated she got out to look at the vehicle and didn't see any damage, so she took a photo of the license plate and would try to report it once she got home. MA Reg TC58LC had several scratches on the bumper of the vehicle.

On 3/1/19 while working N493, myself and N492 Officer Selig were sent to the area of 1354 Washington St for a motor vehicle accident with reported injuries. On arrival, I spoke to Matthew Digregorio DOB7/2/74 ( hereby referred to as operator #1) who was standing on the sidewalk next to MA reg 134TR9 (vehicle #1 ). Vehicle #1 had significant damage to the front and right quarter panels. It was straddled between the travel lanes and did not have the keys in the ignition. It was immediately apparent that operator #1 suffered an injury to his head, as there was a moderate amount of blood and small pieces of glass stuck in his forehead area. I observed operator #1 to be unsteady on his feet, have bloodshot eyes and slurred speech. I could detect a moderate odor of alchoholic beverage coming from operator #1 as I was approaching him, which was apparent from about an arms length away (it should be noted that medics were already en route to the scene at this time). Prior to the medics arrival, I asked operator #1 for a brief synopsis of what transpired. Operator #1 stated that he had just left Paddy's Pub and was driving westbound on Hunter St (which is approximately 1/8 mile from our location) when he collided head on with vehicle #2. Vehicle #2 was not immediately visible upon police arrival, however was pointed out as being located at the intersection of Hunter and Putnam St. I asked operator #1 where he was heading, as Hunter St is a back road not directly in the vicinity of Paddy's. Operator #1 could not answer the question and also remained silent when I asked him how much alcohol he had consumed this evening. At this time the medics arrived on scene. I asked operator #1 for the car keys to reliquish to the tow truck operator, which is standard operating practice. Operator #1 reached into his pocket and then provided me with the keys. Due to concern over operator #1's obvious head trauma, he was then transported to NWH to be evaluated while the investigation continued on scene. Two other parties approached myself and Officer Selig. One was the operator of vehicle #2, MA reg 1RD999. Operator #2 is identified as Luke Ferdinands DOB 5/11/80. The other party who approached is identified as Douglas Dasilva-Pimenta DOB 6/2/86 (Dasilva-Pimenta will now be referred to as operator #3). Operator #2 states that he was taking a right onto Hunter St from Putnam St, when he observed vehicle #1 on the wrong side of the road approaching him head on. Operator #2 states vehicle #1 then collided head on with him, took a right onto Putnam St, and continued onto the Washington St overpass near 1354 Washington St. Operator #3 states he was inside of #5 Hunter St when he heard a loud crash coming from outside. He looked out the window and saw vehicle #1 traveling at a high rate of speed on Hunter St towards Putnam St. Operator #3 observed damage to the right side of his parked vehicle (now referred to as vehicle #3) MA reg 7CAK80. He immediately got in his vehicle and proceeded to follow vehicle #1, which then crashed into vehicle #2 head on. Operator #1 did not stop until it was obvious that operator #3 was following close behind. This ultimately landed vehicle #1 at it's final location near 1354 Washington St. Vehicle #1 and vehicle #2 were not driveable and were towed to Tody's. It should be noted that there was a passenger in vehicle #2 at the time of the collision. Airbags were deployed and medics checked out both operator and passenger. Both declined transport. The passenger is identified as Elizabeth Ferdinands DOB 5/22/53.Ferdinands' version of events coincide with that of operator #2. Upon speaking with Officer Selig it was learned that operator #1 dropped his cell phone several times as he was holding it on scene. Both Officer Selig and operators #2 and #3 observed this. Operators #2 and #3 both expressed concern over the obvious odor of alcohol eminating from operator #1, and the fact that he was unsteady on his feet. I responded to NWH to follow up with operator #1 however he did not wish to speak further. He did express concern over the well being of all parties involved. Operator #1 was issued in hand the following citations: T1268426 - 90/24 OUI Alcohol, 90/24 Leaving the Scene (property damage, vehicle #2), and 90/24 Leaving the Scene (property damage, vehicle #3). T1268425 - 89/4a Marked Lanes violation. T1269216 - 90/24 Negligent Operation of a M/V (mailed after consultation with superiors) All streets mentioned are public ways in the City of Newton. 1354 Washington St will be the address used on this report and subsequent citations due to multiple locations being involved. Citations and paperwork have been placed in the prosecutor's mailbox. An immediate threat form has been filled out and faxed to Mass DOT. *Attention Police Prosecutor for blood work subpoena, if applicable*

On 3/4/19 at approximately 1630hrs, I responded to the front desk for a report of a past hit and run. Operator#1 stated she was involved in an an accident on 2/27/19 at 1436hrs in front of Mango Thai, 115 Elm St. Operator #1 stated she was stopped waiting for the red light at Elm St and Washington when a "new black or gray SUV" pulled out of a parking spot the right of her. The other vehicle sideswiped her passenger side causing a large scratch. Operator #1i stated she honked and attempted to stop the vehicle but the driver pulled away. Operator #1 was only able to identify the driver as a male but did provide me with a plate of MA Reg 9RM445. The plate comes back leased to a 2018 Gray Jeep Compass registered to a Matthew Veteto. I was unable to find any phone number for Veteto. Operator#1 has already filed a claim with her insurance company who advised her she would need a police report. Due to being unable to verify who was driving, a citation was not issued.

On Saturday March 9, 2019, at approximately 0230 hours, I, Officer Brandon, responded to the Weston/Newton line on Commonwealth Ave to assist Weston PD with field sobriety. On arrival Weston PD had concluded field sobriety and determined that the party, Glenn Matot, was not under the influence. Weston PD then explained that they had been contacted by AAA stating that a single vehicle accident occurred in Weston's jurisdiction. After further review, Weston PD and I determined that the accident was in fact in Newton. I then spoke with the operator of MV1, Glenn Matot, who stated that he was on his way home from work traveling westbound on Commonwealth ave in front of the Charles River Canoe and Kayak when he became distracted by a text message. Mr. Matot further explained that while texting, his car drifted to the right causing him to drive up on the snow bank and up onto the guard rail before then falling off back into the road. MV1 sustained major damage to both the passenger side and undercarriage. Mr. Matot did not sustain any injuries and denied any medical attention. AAA arrived on scene and removed the vehicle from the road and Mr. Matot took an Uber home. Two pictures were taken of the damaged City of Newton guard rail and submitted to IT to be attached to the report.

On 03/19/19 at 14:57 hours, I responded to 220 Dedham Street for a past motor vehicle hit and run accident involving a parked car that occurred at 12:26 hours on this date. Dedham Street is a public way maintained by the City of Newton. The owner of M/V # 1 states her vehicle was legally parked in front of 220 Dedham Street facing northbound when she came out and found a note from a witness saying it was struck by a gray Honda Accord bearing MA Reg. # 8512HV. It appears that the operator of M/V # 2 side swiped M/V # 1 as it was passing by on Dedham Street. I observed marks to the driver's side rear door area and the side view mirror was broken. On 03/20/19 at 08:47 hours, I spoke to the witness, Kawtar Bhih, on the phone and she stated that she was parked on the opposite side of where the accident took place. Bhih observed a gray Honda Accord with an elderly female operating M/V # 2 on Dedham Street almost side swiped another parked car before side swiping M/V # 1. She says that the vehicle slowed down after and then continued to leave the scene of the accident. She then took a photo of the above listed registration plate. I attempted to contact the owner of M/V # 2 (Jali Medical Inc Waltham, MA) and the receptionist was not able to provide any information at the time. I was told to call back the next day. I gave the vehicle and registration info to the person inquiring if they could find a possible driver and call back. While working on this report, I observed that the owner of M/V # 2 was involved in a past accident in our system and the operator at the time had an address of 66 Country Club Rd. Newton. On 03/20/19 at approx. 09:30 hours, I went over to this address and observed M/V # 2 in the driveway with an operator inside. After inquiring about the accident, she stated she was driving down Dedham Street when she heard a noise coming from her passenger side view mirror as she had struck something. She then left the scene not thinking it was serious. I observed fresh damage to her passenger side mirror along with other marks along the doors which appeared to be older. Operator was issued in hand MA Uniform Citation # T1441710 for Ch. 90/S24 Leaving the scene of property damage.

MV1 was backing out of her parking spot in the parking lot at 916 Walnut St when she stopped to allow a pedestrian to cross in front of her vehicle. While she was waiting for the pedestrian to cross safely a vehicle backing out of the parking space across from hers backed into her vehicle and left the scene. Operator 1 was able to obtain a photo of the license plate. I then was able to contact operator 2 on the phone who stated she was at 916 Walnut St and "tapped" a vehicle when leaving and then left the scene. No injuries and both vehicle driven from the scene. MV1 sustained minor damage to the passenger side. Operator 2 states there is no damage to MV2. Operator 2 issued and mailed MA Uniform Citation T1442192 for leaving the scene property damage.

All operators state MV3 was stopped in traffic, with MV2 stopped in traffic behind it. Both vehicles were heading southbound on Centre St. MV1 failed to stop behind MV2, colliding with the rear of MV2. MV2 was sent forward from the impact and collided with MV3. No injuries were reported on scene and no tows were needed. On my arrival all parties had exchanged information. After further investigation it was determined the operator of MV2 was driving with a suspended license. I spoke to her via phone and she stated she thought it was cleared up. I advised her that I would be mailing a citation to her (Citation #T1441680) for operating after a suspended license. She was driven away from the scene by a neighbor (Michael Manna, S20209078).

On 4/16/19 I was sent to 25 Chestnut St the Newton Police Annex for a past motor vehicle hit and run. On arrival I was met by LAU, Justin who stated that upon leaving work around 1530 hrs, he observed a fresh scratch to his personal vehicle. The vehicle, MA 9XS415, was parked backed into the marked parking spot against the exterior wall of 25 Chestnut St and closest to the handicapped ramp. This will now be referred to as vehicle #1. The scratch is on the passenger side doors. 25 Chestnut St is equipped with an exterior surveillance system which caught this incident on camera. At 1210 hrs it shows an older white male in a blue/green pickup make contact with vehicle #1 two times as he was attempting to park. The vehicle drove into the parking spot, hitting the passenger corner of vehicle #1's front bumper, then the passenger door, before coming to a stop.The male that exited the pickup had a distinct grey beard and hair. A still frame of the male was shown to employees inside of 25 Chestnut St who immediately recognized and were able to identify him based off of a check he had dropped off earlier. The party is identified as MASTROPIERI, Rocco DOB 2/16/40. I spoke with MASTROPIERI via phone and explained why I was calling. He informed me that he was at the Annex dropping off a check for the NPMA and did not realize that he made contact with vehicle #1 when parking. MASTROPIERI apologized and stated that he would have reported the incident, had he known. MASTROPIERI responded to headquarters where he was issued a written warning in hand for C.O. 19-75 Failure to Use Care in Turning. The Police Annex is a building/lot with public access in the City of Newton.

On 05/10/19 I met with Luiz A. Lopes who stated his vehicle ( Mass Commercial Reg# T83695 White Toyota Tacoma P/ U) was struck while parked in the lot around 07:00. Lopes said he didn't call the Police right away because he wasn't sure his car was struck there. The truck sustained a cracked front bumper and the fog light was popped out. We went inside of National Lumber and spoke with a manager who said he would contact G.M. Greg Assarian and have him look at the security footage. On 05/14/19 at approximately 14:00 the Police Station received a call from Lopes who just got off the phone with Assarian who found some security footage of the incident. I then traveled over to National Lumber and met with Assarian. He showed me the video and it appears that a newer grey four door Chevrolet truck backed into Lopes truck around 06:58 of 05/07/19. The Massachusetts plate is unreadable but an older White heavy set male gets out of the truck and walks into the store. After parking the truck the male can be seen checking his own truck bumper for damage. The male is wearing a baseball hat and sun glasses and cannot be identified on the video. The male does however walk with a noticeable limp. Assarian will show his picture to his employees and contact me if the employees are able to identify the male driver. I called Lopes back and apprised him of my findings. ******************UPDATE**************************************** As of today 05/20/19 I have yet to hear back from anyone at National Lumber so I will end this investigation and label it a hit and run crash.

On 7/9/2019 at approx 1406Hrs while assigned to 497 I responded to level 4A of the Newton Wellesley parking garage at 2014 Washington St for a report of a minor collision. Upon arrival I spoke with the operator of RI Reg FV705, Malorie CURLEY who stated she had parked her new work vehicle in the parking stall was exiting her vehicle to see if she was within the lines when the operator of Ma Reg 69EL83 Laurence HENDY pulled into the spot next to her, opened his door striking her truck, CURLEY stated as she was rounding the car HENDY proceeded to grab his door and slam it into her car several more times. CURLEY stated she confronted HENDY who refused to exchange information with her for the damage to her car and fled. CURLEY took pictures which she will provide in her own crash report. The front bumper of CURLEYS car had several spots where the white paint was peeled back , HENDYS vehicle had the matching white paint on the edge of his drivers side door. Although the damage was minor CURLEY requested a report for documentation as it was her work vehicle. i attempted to contact HENDY but no phone number is listed.

On 7/20/19 at 23:24 hrs I responded to the area of 552 Commonwealth Avenue for a report of a MVA. Upon arrival it was determined MV#1 had struck two unoccupied parked vehicles in the eastbound parking shoulder. On August 1, 2019 at approximately 17:34 hours I, Officer Guarino, responded with Newton Fire and Medics for a report of a single vehicle crash into the building of 335 Boylston St. Upon arrival, the operator and sole occupant of the vehicle was out of the vehicle and had minor injuries. MV1 struck a stop sign and was up against 335 Boylston St, which appeared to have a broken window and minor structural damage. Operator of MV1 said that he was traveling in the right lane of Boylston St (Westbound) when a white Mercedes sedan (unknown model or registration number) turned into his lane and almost struck his vehicle. Operator of MV1 swerved to the right to avoid a collision and as a result crashed into the stop sign on John St. and the side of 335 Boylston St. Witness 1, who was parked at 325 Boylston St. saw the incident and corroborated MV1's story. A second witness, who I was unable to make contact with stopped and gave MV1 his phone number of (508)215-4951 and texted MV1 that he caught up with the possible vehicle, MA Reg 9BT373, which comes back to DESOUZA PLASTERING SERVICES INC. I will attempt to make contact with an operator during their business hours. The owner of 335 Boylston St, Onir Spiegel, was on scene, was given the incident number and was advised to contact her insurance company. MV1 was towed from the scene by Tody's and the operator signed a patient refusal from the Medics. Pictures were taken at the scene of the damage and submitted to IT to be attached to this report. Sergeant McLean notified the DPW for the stop sign and glass in the roadway. Sergeant McLean also left a message for Newton Inspectional Services to come inspect damage to the building at a later time.

Operator of vehicle 1 states she was traveling westbound on Washington St when the vehicle to her right began to enter her lane and then struck her passenger side pushing her onto the raised island. As a result vehicle 1 suffered a flat front driver side tire. Vehicle 2 did not stop and continued westbound on Washington St towards Wellesley. Operator of vehicle 1 was able to follow the other vehicle and got a license plate of MA reg#149DE7 and described the vehicle as a black sedan. AAA responded and fixed vehicle 1s flat tire and she was able to go on her way. I ran the license plate that was given for the other vehicle and it came back to a 2013 black Kia Rio sedan owned by a Dan Kuksin of 24 Bryon Rd apt 2 in Boston. I was unable to get a phone number for Kuksin. Dispatch requested Boston send a unit by to try and make contact and as of finishing this report I have not heard back. Traffic Bureau Update (Officer Gaudet): On Tuesday, July 6, 2019, I attempted to make contact with the registered owner of MV2, Daniel Kuksin (S61105670), with a negative result. A Hit and Run inquiry was mailed to the Kuksin. On 08/07/2019 the owner of MA reg#149DE7 came into headquarters to discuss what happened. With him was his father, Arkady Kuksin who was operating the vehicle at the time of the incident. Kuksin admitted to operating the vehicle in the area at the time of the incident but stated that someone had thrown a rock at his vehicle causing damage. He then stated that it was not thrown but kicked up from his tire. Myself and Officer Gaudet went outside with Kuskin and examined his vehicle and saw obvious damage that corresponded with the damage to vehicle 1. There were obvious signs of fresh paint transfer. I informed Kuksin that I would be updating my report and he would be receiving a summons in the mail for 90/24 leaving the scene property damage. i mailed Kuksin MA uniform citation T1444589.

On 08/13/19 at 14:53 hours, I responded to a hit and run motor vehicle accident located at 244 Needham Street. The operator of MV1 states she was parked in a parking spot to the right of the Splash business at 244 Needham Street. The parking spot is located on Charlemont Street. After parking, she was sitting in her motor vehicle when she heard a bump to her driver's side rear end from MV2 backing up from a parking spot across from her. She says the operator of MV2 turned and looked at her and kept proceeding (NB) on Charlemont towards Needham Street. She then exited her vehicle to try and get the operator's attention to no avail. She took a photo of MV2 which was a black Toyota Highlander bearing MA reg.# R85045. She described the operator as a white female in her forties with possibly dark curly hair. I observed minor paint damage and scratches to the driver's side rear quarter panel area. The operator is unsure if that damage is from the accident. The owner of MV2, Steven Masterson has a residential address of 40 Wild Acres Rd in North Attleboro, MA and a mailing address of 34 Central Street Foxborough, MA. A telephone search revealed a phone number of (269)857-7814 for him out of Aiken, SC and the Foxborough address but the number is not in service. UPDATE: I was provided a phone number for Masterson from N. Attleboro PD. On 08/16/19 at 12:45 hours, I spoke to Sheila Masterson who stated she was driving MV2 at the time of the reported accident. MV1 operator, David Gorman (2017 Mazda Cx3, MA Reg:6TL761), stated that he was traveling from Commonwealth Ave turning right on Lexington St heading NORTH when he realized the vehicle behind him was following him very closely. MV1 continued to drive on Lexington St when MV2 (2015 Toyota Highlander, MA Reg: 6VS151), started to beep at him continuously and went over the double yellow line in an attempt to pass MV1. MV2 was unable to pass MV1 because of oncoming traffic so MV2 steered to get back into his lane. MV2 collided with MV1 in the area of 281 Lexington St. A witness, John Maclellan (off-duty police officer Watertown) who was in the area of Lexington St and Orris St. stated that he heard a vehicle beeping repeatedly and then heard a collision. Maclellan came outside to find MV2 over the double yellow lines facing oncoming traffic. MV2 had damage to its passenger front and MV1 had damage to its drivers side rear. Maclellan told MV2 operator, Renato Ribeirodasilva, to move his vehicle to a safe location on the right side of the road and get out of the vehicle to exchange paperwork with the other driver involved (MV1, Gorman). Maclellan phoned Newton PD to notify of the accident and as he was on the line, he witnessed both parties getting into a heated arguement on the sidewalk outside of their vehicles. MV2 operator, Renato Ribeirodasilva was taken into custody for the following charges: 90/24J - OUI Liquor, 90/24E - Neglegant Operator of a MV, and 89/4A - Marked Lanes Violation. Claudineia called a sober friend to drive her and her three children home to Waltham in Claudineia's vehicle. Claudineia was also under the influence of alcohol.

On 09/04/19 at 08:22 hours, I responded to 1052 Chestnut Street for a three car motor vehicle accident with minor injuries. All four parties involved were shaken up as a result of the accident but signed patient refusals with Cataldo Ambulance One. Operator of MV1 states she was traveling straight ahead (SB) by 1052 Chestnut Street when she observed several vehicles ahead start to proceed through the light. She then heard her phone ring and went to retrieve it by looking down for a second. When looking back up, the traffic directly in front of her did not move yet and she could not stop in time striking MV2 in the rear end. MV2 was then pushed in to MV3. Both MV2 and MV3 were stopped in traffic at the time awaiting to travel through Chestnut Street at Elliot Street. MV1 sustained moderate front end damage to the bumper and grille areas. MV2 sustained heavy rear end bumper and hatch area damage. MV3 sustained minor scratches to the rear bumper. All vehicles were still driveable.

On 9/08/19 Kevin Coston came into NPD Headquarters to report a MV/Ped with Injury that had occurred on 8/23/19 at about 06:15 in the parking lot of Whole Foods (916 Walnut Street). The RP stated that he had left work (Whole Foods) at the back receiving door and was walking to his car, he bent over to take his knee pads off, while he was doing this he was struck by a delivery van that backed into him. The driver took out his headphones and said sorry before driving away. RP stated that he told the driver that he had been hit but the driver just drove away. RP went to hospital (Faulkner) on his own for lower back pain and has not been to work since. RP was able to take picture of the side of the delivery van. The van belonged to Shortstop Distribution Inc (508-655-5948). RP left a message that morning. He was texted by Gedeon (786-352-6695) who stated there was a language barrier and he didn't understand what had happened. RP advised Gedeon to talk to his lawyer or the police. Exact location of accident unknown. Diagram is a estimate. Report printed out and placed in traffic box for follow up. Traffic Bureau Update (Officer Gaudet): On Monday, September 9, 2019 I attempted to make contact with the operator if MV1, Gedeon Duarte, with a negative result. A Hit and Run motor vehicle inquiry was mailed to Duarte. I spoke with the injured party, Kevin Coston, via telephone on September 9, 2019. Coston stated he is an employee at the Whole Foods located at 916 Walnut Street, Newton. Coston stated on August 23, 2019 a black van backed into him as he was kneeling on the ground taking off his knee pads in the loading area of the store. Coston stated he approached the driver who remained in the vehicle to speak with him after he was hit. Coston described the driver as a heavy set Spanish male. Coston stated the operator of the vehicle rolled down his window, took his headphones out of his ear, said sorry, and left the area. Coston stated he contacted the Newton Police to report the accident but was told he needed to come in person to file the report. Coston stated he was unable to come to the station to file the report until September 8, 2019. On Friday, September 13, 2019, I spoke with Gedeon Duarte at the Newton Police Department. Duarte stated he spoke Portuguese and did not speak English. With the assistance of David Sargent from the Language Line (1-800-225-5254) translating, I was to speak with Mr. Duarte in the Traffic Bureau. Duarte stated he was making a delivery at Whole Foods and was waiting for another truck to leave so he could leave the area. Duarte stated a male party (Coston) came up to his driver door and was yelling him. Duarte stated that due to language barrier he was unable to understand what the person was saying to him, but stated the male party was yelling, "Fuck you! Fuck you!" Duarte stated he did not know what was going on and stated he left the area to allow the male party to calm down. Duarte stated he returned a minute later the male party did not want to speak with him. Duarte stated he returned a second time later in the morning and Coston was not there. Duarte stated he received a call from his employer stating Kevin Coston had called for him and he was provided with Coston's phone number. Duarte stated he texted Coston to find out what happened that morning. Duarte showed me a text message exchange from August 23, 2019 at 9:03 A.M. between himself and a party listed as "Kevin Whole". In the text messages, Duarte stated he is the driver of the van and that his supervisor called him. The other party responded and stated "Talk to the police and my lawyer". Duarte stated that he did not understand what happened and that he returned to whole foods. Duarte ended the text exchange with "927535425 my number of policy..." Duarte stated he had no idea he made contact with Coston and stated at the time of the alleged accident he did not hear/see/feel anything otherwise he would have stopped. Duarte's information has been updated in this report to reflect him as the operator/vehicle involved. Duarte also submitted pictures of the rear of his vehicle, a 2008 black Ford E350 (MA: 8NY171), from the day of the accident. Photos were submitted to the IT Bureau. Officer Wade went to the Whole Foods located at 916 Walnut Street, Newton in an attempt to view surveillance footage of the accident. Officer Wade stated the surveillance camera did not capture the incident and the van not viewed at all.

The operator of Vehicle 1 stated he was sitting in Vehicle 1 while parked when Vehicle 2 backed into his front end. The operator of Vehicle 1 stated that after the collision Vehicle 2 left the area without leaving any information. The operator of vehicle 1 described Vehicle 2 as a red Honda Accord bearing MA Reg 365KH6. The operator of Vehicle 1 described the operator of Vehicle 2 as a white female approximately 70 years of age with blonde/white hair. The operator of Vehicle 1 states that Vehicle 2 had minor damage to the drivers side rear bumper. No injuries were reported on scene. No vehicles were towed. An RMV check revealed Vehicle 2's registered owner as Joan Goldman of 53 Bartlett Way Apt 205 Waltham, MA. I was able to contact Goldman over the phone. Goldman identified herself as the operator and owner of Vehicle 2. Goldman states that while she was backing into a parking stall she was going very slow. Goldman states she did not feel any impact and left the scene because she did not think she collided with Vehicle 1. Goldman states she was not injured. Goldman was provided with the information of Vehicle 1 and was advised to contact her insurance company. I also called and provided the owner of Vehicle 1 with the information of Vehicle 2.

The OP. of MV#1 states he was in a parking space and trying to correct his position within the lines. He pulled out and then when he was about to drive forward, MV#2 suddenly reversed into him. The OP. of MV#2 states she was exiting the parking lot due to it being full. As she was driving forward, MV#1 pulled out of a spot and collided into her. There is supposedly a witness by the name of Jean Charles working for VPNE Valet that observed the accident. He has left for the day and does not have a cell phone or a house phone. Manager on duty states only corporate headquarters 617-451-1393 has information on him. I checked with hospital security for surveillance footage and observed MV#2 passing MV#1. MV#1 then started backing out of his space but MV#2 started reversing. Both vehicles ended up colliding with each other at an angle. On 11/6/19 at approximately 0610 hrs I responded to 15 Needham St., the National Lumber for a MVA involving a pedestrian. Upon arrival I spoke to the pedestrian who was lying on the street who stated he landed on his back and couldn't feel if anything was hurting. I advised him to stay put until the medics responded. The pedestrian was wearing brown pants and a black jacket and stated he was listening to his head phones when he attempted to cross the street. Where he attempted to cross was down Needham St from the main intersection and there was no crosswalk. He attempted to cross from the National Lumber side crossing the S/SW travel lane first. I then talked to the operator of MV#1 who stated he turned right onto Needham St from Winchester St and didn't see anybody attempting to cross until the last second. At this time he stated he jammed on his brakes and saw pedestrian on the hood of his Pickup before he fell to the ground. There was minor damage to the hood of the vehicle on the passenger side. The pedestrian was evaluated by the Medics and transported to NWH for further evaluation. Pictures were taken on scene and turned over to the IT bureau.

On 11/30/2019, while assigned to N494, I, Officer Conary, and other units, responded to Charlesbank Road at Nonantum Road for a single motor vehicle crash into a tree. Upon arrival, I observed a white mini van MA registration 5DV184 with heavy front end damage into a tree on Charlesbank Road, a public way in Newton. The operator of the motor vehicle, CORDERO, Ericka, was sitting in the back seat of the vehicle and was talking on the phone. CORDERO's primary language was Spanish. I asked CORDERO if she was okay and she said yes and I asked CORDERO if she was driving the car and she said yes. CORDERO also told me that she did not have a license. MA State Police were on scene and were able to asist with translating. Medics arrived and CORDERO was transported to Saint Elizabeth's Hospital. CORDERO's son and husband arrived on scene and I was able to confirm her information. Both stated that CORDERO did not have an active license. Newton Dispatch ran all information for CORDERO and confirmed she was unlicensed. CORDERO will be charged with MGL Ch.90/10 Unlicensed Operation of a Motor Vehicle and 89/4A Marked Lanes Violation, Uniform MA Citation T2079476. Due to CORDERO being transported to the hospital, the citation will be mailed to her active address. Pictures of the incident were taken and uploaded to IT accordingly. No further incident to report.

On 12/19/2019 at 14:43 hours Newton Police dispatch received numerous phone calls about a white male whose face was covered in blood and was riding on an electric motorized scooter south bound on Walnut St near number 100. Walnut St is a public way in the city of Newton. Approximately two tenths of a mile south 100 Walnut St, Ofc Garcia stopped the male on the scooter. Ofc Garcia stated that the male on the scooter was evasive when answering his questions and claimed that he did not realize that his face was covered in blood. The male on the scooter was identified as Gregory David Brown. Brown was transported to Newton Wellesley Hospital for treatment. Prior to Brown being transported I was dispatched to the area of 100 Walnut St for a report of a male on an scooter who had crashed into a south bound vehicle in traffic on Walnut St and another vehicle that was parked in front of number 73. Upon my arrival I spoke with Penni Rochwerger and Nikita Espinal. Rochwerger stated that she was driving south bound on Walnut St across from 100 when she activated her vehicle's four way flasher and stopped her vehicle to let Espinal who was the front seat passenger out. Rochwerger stated that south bound traffic stopped behind her vehicle after she came to a stop adjacent to Espinal vehicle that was parked facing south in front of 73 Walnut St. Espinal stated that she checked over her right should to see if it was clear to open her door and it was . Espinal stated that as she opened her door to exit a male on scooter crashed into the door and fell to the ground. Both Rochwerger and Espinal stated that the male on the scooter did not provide his name or address and continued south bound on Walnut St with his face coved in blood. Rochwerger's vehicle sustained damage to the front passenger side door that would not closed all the way. Espinal vehicle also had damage to it, the rear driver's side lower quarter panel was struck. I responded to Newton Wellesley Hospital to speak with Brown. When I arrived and spoke with Brown he was evasive with his answers . When I asked Brown what had happened he gave conflicting answers and contradicted himself. Brown stated that he was riding his scooter on the roadway south bound on Walnut St be could not recall any other details. When I asked Brown if he was wearing a helmet as required by MGL 90/1E he stated he couldn't recall. Rochwerger, Espinal and Ofc Garcia all stated to me that they did not see Brown wearing a hamlet. There was an unconfirmed post by a person named Tricia Marucci to the Facebook group Newton Parents stating that at 14:45 hours on 12/19/2019 saw Brown hitting a defect in the roadway in the area of Crafts St by California St that caused him to loses control of his scooter and fall face first onto the pavement. In her post Marucci stated that Brown was cover in blood from facial lacerations, and that he refused medical aid and continued to ride his scooter south bound towards the entrance to Walnut St. I left a message for Marucci to contact me and as of the writing of this report she has yet to do so. While at the hospital I ran Brown's name and date of birth via the MA RMV data base in order to issue him a citation for not wearing a helmet. The query of Brown via the MA RMV data base reviled that Brown's MA driver's license statutes was suspended. On 07/11/2018 admitted to a finding of sufficient facts to one count of MGL 90/24 OUI liquor. Brown received a Continued With Out Finding for 18 months. In addition Brown pleaded guilty to an additional count of MGL 90/24V Child Endangerment While OUI. Brown received a 90 day sentence in the Middlesex House of Correction which was suspended for 18 months. Brown also received a one year loss of his driver's license. Brown is on active probation out of Newton District Court until 07/21/2020. Biased upon the statements made to me I issued Brown MA Criminal Application T 2012526 for violations of MGL 90/24(2) Leaving The Scene Of An Accident Property Damage , MGL 90/1E Operation Of A Motorized Scooter Without A Valid License and MGL 90/1E Operation Of A Motorized Scooter With Out A Helmet. As of 12/20/2019 Brown was discharged from Newton Wellesley Hospital.

At approximately 1645HRs on Tuesday, 01/07/2020, I was dispatched to 325 Boylston St. (Sunoco Gas) for a report of a hit and run motor vehicle crash. Upon arrival, I spoke with the operator of MV1 (MA Veteran:J706), who stated that as he sat stationary in his vehicle just before entering pump: 3, MV2 backed up from pump: 1 and struck MV1 in the front bumper, despite him activating his horn and keeping it active as she continued to back up. The operator of MV1 then approached the driver's side door of MV2 to exchange information, however the operator of MV2 drove off; the operator of MV1 provided MA Pass: 8SV842 being attached to MV2 and I noted damage to the front bumper of MV1. I spoke to witness 1 via phone and he concurred with the operator of MV1's account of the incident. It should be noted that witness 1 could see the crash well from his vantage point by pump: 2. The operator of MV2 could not be reached by this officer for comment. Natick PD responded to the operator of MV2's residence and observed damage to MV2's rear bumper. They requested the operator of MV2 contact me via phone and she stated she would; Natick PD did not get a contact number for her. Natick PD informed us to expect her call and a few minutes later, dispatcher Kelly stated that she received a call from the operator of MV2, who immediately hung up once the call was answered. It should be noted that since she called on the business line and immediately hung up, we were not able to retrieve her phone number as the system does not store it. Due to the operator of MV2 failing to provide her information after the crash/fleeing the scene and her not being cooperative with my investigation, I have issued her by mail MA Criminal Citation:T2013367 for violating one count of MGL. Ch. 90 S. 24 (Leaving The Scene Of An Accident After Property Damage). Note: the clerk at Sunoco stated that the cameras at the pumps do not store footage. **Update** At approximately 1540HRs on Thursday, 01/07/2020, I spoke with the operator of MV2 via telephone. She contacted the Newton Police Traffic Bureau and provided her phone number, who in turn forwarded it to me. She stated to me that on the night in question, she was at the pumps and the pump she initially wished to use (pump: 1) had a sign on it to pay inside. She then elected not to use said pump and backed her car up and heard the operator of MV1 activate his horn. She stopped and was unaware that she struck MV1. When she saw the operator of MV 1 exit his vehicle, she assumed he was going to pump gas. She then stated she left the gas station because she was frustrated and did not intend to leave the scene of an accident. I advised the operator of MV2 that since I was unable to speak with her that night, I had already issued her MA Criminal Citation:T2013367 for violating one count of MGL. Ch. 90 S. 24 (Leaving The Scene Of An Accident After Property Damage) via mail and advised her how to proceed.

Vehicle # 1 was parked in a parking spot at 286 Waverley Ave. Vehicle # 2 backed out of parking spot next to vehicle # 1 and when tried to make the turn after backing up, it struck Veh # 1. This was witnessed by two witnesses who ran outside and the car was gone. They were able to give dispatch plate of vehicle # 2 which matches description of a tan SUV with a woman driver. Due to very minor damage (small scrapes), Oper # 2 will not be charged with leaving the scene, she may have not been aware of damage) and RP (owners daughter) agreed that she just wanted it documented. Attempted to locate MV #2 at her home but was unsuccessful. Also tried phone number given and it wasn't answered and I wasn't able to leave a voicemail.

MV1 stated he was travelling w/b on Charlesbank Rd when MV2 was travelling e/b and "entered my lane and our mirrors hit" causing minor damage. MV1 further stated that when he asked the operator of MV2 to exchange information the operator stated "why? you hit me" and sped off towards Brighton. Operator of MV2 is described as a white male in his late 40s with a goatee. I will attempt to follow up with the owner of MV2 on my next tour of duty. On 2/8/20 @ approximately 1400 hrs New Bedford PD responded to MV2s residence however neither he or his vehicle were there. NBPD left a note for the owner to contact me. It should be noted NBPD does not have a phone number for this party. Traffic Bureau update (Officer Gaudet): On Monday, February 10, 2020, I attempted to contact the registered owner of MV2 with a negative result. A Hit and Run inquiry was mailed to the registered owner of MV2, George Coelho (S65129960).

The operator of MV#1 stated he was travelling northbound on Chestnut St when MV#3 struck his vehicle travelling southbound on Chestnut St. MV#3 continued southbound on Chestnut St and struck MV#2. MV#3 continued southbound on Chestnut St and left the accident scene. MV#1 sustained no visible damage due to it being a large steel framed garbage truck. There were no reported injuries to the operator of MV#1. The operator of MV#2 stated she travelling northbound on Chestnut St directly behind MV#1 when she observed MV#3 struck MV#1 then striking her vehicle.MV#2 sustained heavy damages to its driver's side mirror. There were no reported injuries to the operator of MV#2 or its passenger. MV#3 was described as a white colored small sized "Sprinter van" type vehicle with heavy driver's side damages. The operator of MV#3 was reported to be a young white male and was texting and driving. A radio broadcast was given out to surrounding cities and towns and a canvass of the surrounding area showed negative results for MV#3.

On Wednesday 02/19/2020, at approximately 1500 hrs, operator of MV1 stated she was traveling eastbound on Newtonville Ave, right before Walnut Street, a dark colored pick up truck was traveling behind her. Operator of MV1 stated she was driving below speed limit due to the approaching intersection at Walnut street when MV2 went into the eastbound traveling lane attempting to go around her. While attempting to go around MV1, MV2 side swiped MV1 causing the driver side mirror to break off causing moderate damage to the driver side of her vehicle. Operator of MV1 stated the pick up truck then stopped in front of her and the operator, an adult male began to yell at her then drive off. Operator of MV1 stated she got back in her car and did not get any information and did not have her cell phone to call the police. Operator of MV1 stated she was able to drive her vehicle home that day. No further description of the pick up truck was provided.

The operator of vehicle 1 stated she was traveling West on the Washington Street bridge over the Mass Pike when vehicle 2, a gray SUV attempted to change lanes, as this happened the front passenger side quarter panel of vehicle 2 came in contact with the rear drivers side quarter panel of vehicle 1. This caused minor scratches to vehicle 1. The operator of vehicle 1 attempted to pull over and exchange information with the other driver. The operator of vehicle 2 continued driving away and got onto the Mass Pike, during this process the operator of vehicle 1 and the passenger of vehicle 1 were able to get the license plate number of vehicle 2 which was NY REG:T797800C. In an attempt to locate and identify the driver of vehicle 2 I ran NY REG T797800C which comes back registered to a short term rental company called Flexdrive. I called a phone number on their website 8663539399 which is listed as the number to call if you are in an accident with one of their vehicles. I was only able to speak to a service technician, a Tom Louis of ARI Fleet Service 18002272273, who told me the vehicle was out of the Flexdrive North Dighton Mass hub located at 123 Williams Street North Dighton MA 02764.Tom also mentioned the location would most likely not be open again until Monday. He did not have any other information regarding who may be operating the vehicle. I have attempted to find contact information on this North Dighton location which I have had negative results so far. I have also emailed the Flexdrive customer service department. To be followed up on. Traffic Bureau update (Officer Gaudet): On Tuesday, February 25, 2020, I made contact with the registered owner of MV2, Flexdrive. The representative from Flexdrive stated MV2 is registered to the company but Lyft uses their vehicles for ride sharing. I contacted Lyft and spoke with a party named Jonas from their support team. Jonas stated he would make contact with the operator of MV2 and ask them to contact me to give a statement. Lyft stated they would also conduct their own internal investigation. On Wednesday, February 26, 2020, I spoke with the operator of MV2, Gary Procaccini (S36066201). Procaccini stated he was operating a 2019 Santa Fe (NY Reg: T797800C) as a Lyft driver on the evening of February 22, 2020. Procaccini stated he was traveling on Washington Street (W) towards the Massachusetts Turnpike entrance in West Newton. Procaccini stated he was in the wrong lane and looked in his mirror before attempting to merge into the correct lane. Procaccini stated to the right of his vehicle he observed a vehicle "flying" towards the entrance to the Pike. Procaccini stated he came to stop to allow the vehicle to pass. Procaccini stated the vehicle passed at a high rate of speed and he continued on his way onto the Pike (E). Procaccini stated he had a passenger in the vehicle using Lyft servies and there were no signs that a crash had taken place. Procaccini's information has been added to his report to reflect him being the operator of MV2. I then spoke with Peter Houser who is a Driver Services Representative with Lyft. Houser stated he had just visually inspected the vehicle alleged to have been involved in the crash. Houser stated the vehicle was in "pristine" condition and he observed no damage to the front passenger side quarter panel of MV2. Houser e-mailed me pictures of the vehicle. I observed no damage to MV2. Those pictures were submitted to the IT Bureau.

Logue also stated that his cell phone screen was cracked due to hitting the pavement. Both parties refused medical treatment. Pictures were taken of injuries and submitted to IT. On May 15, 2020 at approximately 22:26 hours Officer Stake and I responded to 163 Suffolk Rd for a hit and run that had just occurred. Upon arrival, we observed MV1,a black Cadillac Escalate with heavy rear end damage parked facing westbound in front of 163 Suffolk Rd. MV1 also was had damage to the passenger side due to it being pushed into a utility pole. MV1 was parked and unoccupied at the time of the crash. I spoke with the residents of 170 Suffolk Rd, who said that they heard the crash and went outside and saw a dark colored Jeep Wrangler leaving the scene at a high rate of speed. They described the Jeep Wrangler as a "newer" model, possibly a four door with a hard top. The residents of 163 Suffolk Rd have a camera system and will view the footage at a later time. Units checked the area for the Jeep Wrangler but were unable to locate it. Todys's responded and removed MV1 from the scene. **Update by Ofc. Hamann** At 0600 hours, Dispatch advised me that a male party that was involved in an accident last night (5/15/20). Brett Catlin stated that he was driving home from CVS last night and admitted to looking down at his phone and then had crashed. He stated after he crashed he drove to his home, which is 4 houses down. He stated he had called the owner of the vehicle and apologized and stated he was going to report it in the morning. I advised him that there will charges pending.****

Mv#1 owner stated he parked his Mv on Lakewood Rd (opposite #23 driveway) at about 1000hrs this date. Owner stated he returned to his unoccupied parked Mv at about 1045hrs to find fresh light moderate driver's side front end damage with light scrape marks. A witness from #32 left a note indicating a beige or tan Range Rover unknown plates (Mv#2) backed into that front left bumper. I raised the witness who further stated she was on her porch and heard a noise but didn't see a collision. She stated it (#2) backed out from the driveway and observed it perpendicular to #1 trying to maneuver away from it and drive away. I raised occupant of #23 and spoke to #2 operator who was not on scene by cell phone. #2 operator was unaware of any collision. #2 texted pictures of his Mv's rear bumper. One picture appeared to show a light colored scrape on driver's side upper rear end. Mv#1 is dark blue. Mv#2 is bronze. Both parties agreed to exchange information. Both parties were advised the matter would be documented and to proceed further with their respective insurance companies.

On June 13, 2020 Officers responded to 129 Parker St for a report that a vehicle crashed into a parked car. Upon arrival we spoke with the operator of MV1 who was being evaluated by medics. The operator of MV1 declined further medical attention for leg and back pain. The operator of MV1 informed me that he was traveling southbound on Parker St. when he became distracted by his cell phone GPS falling on the floor. He went to retrieve the item and struck the rear of MV2 which was parked in front of 129 Parker St. The vehicle owner of MV2 was on scene and was advised of the situation. Due to the damage, both vehicles were towed from the cent by Tody's towing.

On 6/29/2020 at approx 0755Hrs while assigned to 497 I responded to the area of 18 Annawan Rd for a report of a hit and run. Upon arrival I met with resident of 18 Annawan Rd , Andrea Epstein, and her boyfriend Isaiah Mohammed . Epstein stated her fathers vehicle ,MA Reg 315SA6 was parked on the street in front of the home. Isaiah stated he was looking out the window when he witnessed a large black flatbed delivery vehicle backing up attempting to make the turn from Annawan to Waban Av when it struck the Epsteins vehicle, Isaiah stated he made eye contact with the white male driver who realized he had struck the car than accelerated away turning left onto Waban Av. They attempted to get a plate and found possible partial plate of 29866 and saw "Eastern...." imprinted on the side. Isaiah and Andrea related a neighbor whose name they didn't know followed the truck when it fled and got a picture of the license plate and they were awaiting the photo being forwarded to them, they were also going to forward photos from the Ring doorbell that captured the truck. I checked the area for construction sites with recent drop offs. I contacted Andrea who stated she had spoken with her neighbor at 70 Edgfield Rd, Patrick Nuzzi who stated he had just had a delivery of fencing delivered by Eastern Fence of Medford NY and upon seeing the photo from her Ring doorbell confirmed it was the same truck due to the empty spot on the flatbed where his fencing had been . I spoke with Patrick Nuzzi who stated he would get in touch with his contractor Amazing Gardens to get the truck drivers info and registration info. I was contacted via phone by an employee of Eastern Fencing LLC from 631-698-0975 Bob Eggerman , who stated he would review the dash cam footage and forward the operator and registration info. 6/30/2020 driver and reg info forwarded by Eggerman, report updated.

On 7/3/20, I met a party in the lobby of the NPD for a past hit and run report. The RP, Aleksander Duri ( MA Reg: 2LG632) stated he was parked in the Wegmans parking garage on 6/27/20. When Duri returned to his vehicle there was a note on it that said "I just saw a car hit your driver side back bumper. Teenage boy was driving, license plate 159XT5 Blue Subaru outback. If you have damage and need a witness you can call me [phone number in witness section]." MA Reg: 159XT5 comes back to a 2013 blue Subaru Outback registered to Emily Franklin-Strauss. In the census it lists three males who reside at the address in the same age group (DOB of 11/8/2001, 5/20/2004 and 7/2/1999). I attempted to call the phone number on file with negative results. There are cameras in the Wegmans garage however it did not capture the accident. Traffic Bureau update (Officer Gaudet): On Monday, July 6, 2020, I attempted to make contact with the registered owner of MV2, Emily Franklin-Strauss (S56184971), with a negative result. A Hit and Run inquiry was mailed to Ms. Franklin-Strauss. On Wednesday, July 15, 2020, I spoke with the operator of MV2, Samuel Strauss (S90135443), via telephone. Strauss stated he was operating MA: 159XT5 at the time of the crash in the parking garage. Strauss stated he was backing his vehicle up and heard a sound. Strauss stated he got out of his vehicle and looked for damage on the parked car he backed up into. Strauss stated he stayed on scene for 7-10 minutes and left after he observed no damage. Strauss stated a claim has been filed and insurance is now involved. Strauss stated he is an inexperienced driver and did not know what to do in a situation like this. Strauss was advised that if he is invovled in a crash in the future, he should contact the local police department in the city the the crash occurs in so the police can help faciliate an exchange of information. Strauss stated there was another passenger in the vehicle with him at the time of the crash named David Banteekhan. Strauss could not provde any other information for this party except that he lives in Newton and his Snapchat name is DDavid5935. I could not find any other information on this party. No injuries reported. Strauss is a student at Newton South High School. No futher action will be taken.

On 07/28/2020 at 1221hrs I responded to the area of 23 Oak St for a motor vehicle crash involving a pedestrian. The involved pedestrian had left the area prior to my arrival. The vehicle that was involved in crash was also not on scene. The crash occurred on 07/28/2020 at approximately 0900hrs in the driveway of 27 Oak St. Prior to the crash the involved vehicle described as a 1991 Chrysler TC Convertible color Red bearing MA Antique Reg D2935 ( Active) was located in the driveway of 23 Oak St. The vehicle was facing toward the rear of the driveway. The vehicle then rolled in reverse easterly across the common driveway and struck a pedestrian who was located in the driveway of 27 Oak St. After contact with the pedestrian the vehicle came to final rest in the driveway of 27 Oak St. As a result of the impact the pedestrian sustained a minor injury to her left knee. There were no occupants in the involved vehicle at the time of the crash. It is unknown at this time if the vehicle sustained any damage. The pedestrian left the area prior to this crash being reported to NPD. The vehicle was removed from the area prior to the crash being reported to NPD. I then spoke to the owner of the involved vehicle Marc Dumoulin who was on scene and was the reporting person. Dumoulin stated that the crash occurred today 07/28/2020 at approximately 0900hrs. Dumoulin states that the involved vehicle described as a 1991 Chrysler Convertible was having issues due to a gas tank problem and he believes it wasn't going to pass inspection. Dumoulin stated that he took his vehicle for a final test ride around the block to decide if he wanted to keep it or not. Dumoulin states that after operating the vehicle he decided to salvage the vehicle with CJ Auto in Watertown, MA. Dumoulin states that before going to CJ Auto he decided to stop back at his home first. Dumoulin states he then parked the vehicle at this driveway and got out to go in the house. Dumoulin states that he believes the vehicle was running at this time. Dumoulin states that as he was walking toward his house he observed his vehicle rolling in reverse towards his neighbor's driveway. Dumoulin states that the vehicle then rolled into his neighbor's driveway near his neighbors fence. Dumoulin states that the pedestrian was outside near the fence when his vehicle rolled into his neighbor's driveway.Dumoulin states that he did not initially report the crash because he did not believe that his vehicle had come in contact with the pedestrian. Dumoulin states that he then removed the vehicle from his neighbor's driveway and brought it to CJ Auto for salvage. Dumoulin states that when he returned to his residence he became aware that the pedestrian may have been struck by his vehicle and had a minor injury. Dumoulin states that upon hearing this he called NPD. At this time I was met by Nina Levin who is the homeowner of 27 Oak St. Levin stated that she did not observe the crash but a contractor working on her house did. Levin stated that the pedestrian involved was named Lucero Flores-Roman and that Flores-Roman was outside attending to some potted plants when she was hit by the involved vehicle. Levin stated that Flores-Roman received an injury to her left knee. Levin stated that she took photo's after the crash of the involved vehicle and of Flores-Roman's knee. Levin provided me with these photo's which I then submitted to the IT Bureau. Levin provided me with Flores-Romans contact phone number. Dumoulin also provided his insurance information to Levin so that she could relay the information to Flores-Roman. I then spoke with a witness Gleb Andreev who was a carpenter working at 27 Oak St. Andreev states that he was inside of 27 Oak St working on a first floor window facing the driveway. Andreev state he observed the involved vehicle roll into the driveway and come to rest near the pedestrian. Gleb stated he then went out side to assist the pedestrian. Gleb stated he believed the vehicle hit the pedestrian when it rolled into the driveway. At 0230 hrs I was able to speak with the involved pedestrian Lucero Flores-Roman. Flores-Roman states that around 0900hrs today she was in the driveway of 27 Oak St attending to some plants near a wooden fence. Flores-Roman stated that her back was turned towards the area of 25 Oak St, she then on a hunch turned around and observed the involved vehicle in reverse moving towards her. Flores-Roman stated that she turned her body quickly to avoid the vehicle but the vehicle's rear end passengers side made contact with her left knee. Flores-Roman stated that she sustained a minor injury to her left knee which included some bruising and swelling. Flores-Roman states that after crash she did not think she needed an ambulance but wanted to go home to rest her knee. Flores-Roman was then provided with Dumoulin's insurance information. No other injuries reported on scene. Photo's were taken on scene and were submitted to the IT Bureau. On 07/29/2020 I responded to CJ Auto's Salvage Yard located at 43 White Pond Rd Stow, MA which was the present location of the involved vehicle. Upon arrival I observed the involved vehicle which did not appear to have exterior damage. Photo's were taken of the vehicle and were submitted to the IT Bureau. An RMV check of vehicle revealed that a title was issued to Dumoulin on 03/25/2020. The vehicle has not been inspected by a MA inspection station. The vehicle title has been signed over by Dumoulin to CJ Auto for destruction. At this time it is unknown if the crash was caused by mechanical failure with the vehicle or human error.

On Sunday 8/2/20 at approximately 701 hours while assigned to marked unit n499 I was dispatched to the rear of the Lifetime Center (300 Boylston Street) for a single vehicle motor vehicle crash. Upon arrival I observed a Black Honda CRV with significant front end damage next to a dumpster in the shoulder of Florence Street behind the Lifetime Center. Dumpster was placedt there for the ongoing construction at the Lifetime Center. I spoke with the operator of the CRV, LYSGAARD Ingrid, who states she was frustrated by all the noise being made by the construction workers. Ingrid states she drove to the site to take pictures of the workers whom she believed to be working in violation of Newton's city noise ordinance. She pulled over to the side of the road to take a picture with her cell phone, but didnt realize her foot came off the brake pedal causing her to strike the dumpster. Fire and Medics also responded to the crash and ingrid signed a patient refusal after being evaluated by the medics Tody's responded and towed the vehicle. I stayed on scene until the vehicle was removed and Ingid's son came to transport her hiome.

At approximately 1735HRs on Saturday, August 29, 2020, I was dispatched to 14 Elliot St. (Sunoco Gas Station) for a report of a two car MVA with one party refusing to exchange RMV information. Upon arrival, I spoke with the operator of MV1 (MA Pass: 6GT965), who stated that he was stationary in his vehicle at pump 3, when the operator of MV2 reversed into his vehicle after refueling at pump 5, which is directly in front of pump 3. The operator of MV2 got out of her vehicle and began to yell at him and refused to give him her RMV information, claiming he hit her. The operator of MV1 stated that he called the police and the operator of MV2 subsequently left the scene prior to my arrival. Officer Keefe (N495) attempted to raise the registered owner of MV2 (MA Pass: 2KWK11) at her residence with negative results and dispatch was unable to find a cell phone for her. I was able to speak with witness 1, who concurred with the operator of MV1's version of events. After speaking with witness 1, the operator of MV1 and viewing his video he recorded on his phone showing a party believed to be the owner/operator of MV2 entering her vehicle (Caucasian Female with the same light/strawberry blonde hair, as seen in her RMV photo), I have issued her MA Criminal Citation T2015042 for violating one count of MGL Ch. 90 S. 24. (Leaving The Scene Of Property Damage) and mailed it to her address. Note: the cameras at the gas station are not operational at this time and a copy of the aforementioned video has been forwarded to IT to be attached to this report.

I responded to the area of 63 Pierrepoint Rd for a report of an MVA single car crash into a tree. I responded to the area at approx. 1752 hrs 09/19/2020. I was advised by NPD Dispatch that the vehicle was unoccupied and that the vehicle has been there all day according to the anonymous reporting party.. When I arrived on location I observed MA Reg. 31542 off the roadway facing westbound directly in front of a tree. This was located directly in front of the residence of 63 Pierrepont Rd. The vehicle was unoccupied with heavy front end damage and front and side airbag deployment. The windows were left 2/3 of the way open and the cars ignition key was left on the driver's seat. The vehicle had collided into a city tree. There was debris from the vehicle at the base of the tree. The tree had visibly been hit but there was no damage to the tree by my observation. I contacted the vehicle owner Edward G. Murray Jr. He confirmed that he had knowledge of the crash. His son Edward G Murray III was the operator at the time of the crash. Murray III had a female passenger Diana Moreno 07/12/2004. I spoke with both Murray Jr. and Murray III via telephone. Murray III stated he was driving following his GPS and suddenly an animal which he believed to be a small dog suddenly ran in front of the car. Murray III swerved to avoid hitting the animal and drove off the road and collided into the tree. He stated the crash was at approx. 0950 on 09/19/2020. I also spoke with Diana Moreno (passenger), via telephone. She could not tell me what preceded the crash as she was looking at her cell phone just prior to the crash. Murray III and Moreno confirmed they were not injured as a result of the crash. Murray III contacted his father and informed him of the crash. Neither party contacted police. Murray Jr contacted AAA to respond advising the vehicle needed to be towed. Murray Jr. stated he was not further contacted by AAA and assumed that the vehicle was towed. Murray III and Moreno left the vehicle parked off the roadway as AAA was not immediately responding. I photographed the crash scene. The photo memory disk has been submitted to NPD IT bureau. The vehicle was towed by Tody's Service Inc.

Several home health aides rotate taking care of the homeowner of 159 Charlesbank Rd, one in particular was working during the day on 9/30/2020. I have made a concerted effort to obtain such individual's name and phone number, however I still do not have that information at this time. No witness's or surveillance cameras in the area of 159 Charlesbank Rd.

On Tuesday October 13, 2020 at approx 1323 hours I went to the area of Anthony Rd at Anthony Cir for a report of a past hit/run that occurred at approx. 1215 hours in the westbound lane of 697 Washington Street. Upon arrival I met the operator of v1 Ms. OLGA KHOLODENKO, she stated she was traveling westbound in the left lane of Washington Street around the Whole foods area which is 697 Washington Street. She stated while traveling a red truck with a red trailer was on her right side traveling along side her, and while traveling the trailer had "swung" into her passenger side of her vehicle. She stated the truck/trailer kept traveling and she followed the trailer and took pictures while driving. She then went home and called her insurance company with the picture of the registration plate and they gave her the information which brought them at Anthony Rd at Anthony Cir, where the registered owner lived. The truck / trailer was not at the location. The trailer is MA trailer reg # C39390 a 2013 Brav trailer color red registered to CIRO CARBONE AND SONS INC 12 ANTHONY CIR NEWTON MA. Her vehicle is a 2020 Kia utility color white MA reg. 6ZG351. I observed some red transferred paint on her passenger side from the rear door to the front door. I advised them of process and I would look into the trailer. I was able to find the truck/trailer on California St at Dalby Street. I looked around the trailer and observed a minor scratch on the driver side of the trailer near the front of the trailer. I contacted the owner who due to the heavy rain all day sent home his employees. I will update oper 2 statement when I talk to him. At this time it appears with the trailer scratch being minor a reasonable person would have not known. Update: Monday October 19, 2020 I spoke with the oper of v2 via phone, he stated he was on Washington Street near Whole Foods traveling westbound with the truck/trailer. He stated he was traveling in the right travel lane. He stated the truck/ trailer was always in the proper slow lane and didn't know about the contact until I notified him. I advised him of the crash process.

On Tuesday, October 27th 2020 at 18:40 hours, Myself N-499 and Ofc Crowe N498 responded to 546 Parker St for a single vehicle accident into a city tree. Upon arrival, I identified Mass Reg # 3GDM90, 2015 LEXS RX350, Color Gray had crashed into a City Tree. The airbags had not been deployed. The operator was no longer in the vehicle upon my arrival. He was identified as registered owner Brian Mcneill. Mcneill admitted he had operated the vehicle. Mcneill stated he was driving North on Parker St toward RT 9 headed home to Wellesley. Mcneill stated he was on his phone texting and lost sight of the road. Mcneill's vehicle veered off the side of the road and side swiped a City of Newton Tree causing heavy right side damage and loss of the front right tire. Mcneill's wife came to the scene to pick him up and drive him home. The set of golf clubs in the trunk of the vehicle was documented in the MV Inventory Crash Form. Mcneill suffered no injuries and signed a patient refusal with the medics. Tody's towed the vehicle. Mcneill was given MA Uniform Citation # T2013042 for the amount of $ 210 for M.G.L Ch 89/S4A Marked Lanes Violation and M.G.L Ch. 90/S13B Texting While Driving Violation. Pictures were taken of the accident and submitted to Newton IT Bureau.

On Thursday 10/29/20 at approximately 0541 hours while assigned to marked unit n499 I was dispatched to the area of 130 Vine Street for a report of a single vehicle roll-over crash. Upon arrival myself and n496(Ofc. Howes) observed a white SUV on its roof facing westbound in the eastbound lane of Vine Street (a public way owned, operated and maintained by the City of Newton). Upon our arrival Newton Firefighters had extracated the operator, (FOSTER Isabelle), who was sitting on a large rock in the front yard of 130 Vine Street. FOSTER was visibly upset, but didnt appear to have any major injuries. I asked FOSTER what happened and she stated she was travelling from Westgate Rd in Boston on her way home to Wellesley. She took a right turn onto Vine Street from LaGrange Street and began travelling westbound. FOSTER stated that she was travelling too fast and didnt realize how the road curved causing her to veer off the road and strike a large tree in the front of 130 Vine Street. FOSTER was travelling at a speed fast enough to cause the vehicle to flip onto its roof in the opposite lane of traffic. FOSTER admitted to me she was travelling too fast on the road. Vine street is a winding road that is not very well lit. Speed limit on the road is 20mph and its estimated FOSTER was travelling at least 30-45mph. The speed coupled with FOSTER's lack of familiarity with the area and her inexperience driving were all contributing factors to the crash. FOSTER was issued Massachusetts Uniform Citation T2012909 with the following civil motor vehicle infractions; C90 s18 SPEEDING (Posted) C89 s4a Marked Lanes Violation. FOSTER was transported to Beth Israel hospital by the medics and I was unable to issue the citation in hand at the time of the crash. Due to FOSTER's age (17) I also wanted to notify her parents and meet with them and FOSTER upon her release from the hospital. At approximately 1700 Hours I met with Scott Foster(Father) in person, Jodi Foster (Mother) and Isabelle foster via cell phone (speaker phone). I discussed the charges with all parties and explained the citation to Isabelle. Citation was given in hand to her father, Scott Foster and brough the citation to Isabelle.

The owner of MV#1 had parked his vehicle at 10:03 in front of 1269 Centre St and returned to his vehicle at 12:00 to discover that it had been struck. Attached to the windshield of MV#1 was a handwritten note from a party that witnessed the accident. MV #1 sustained moderate damages to its front driver's side bumper. I spoke to the witness via telephone and he stated he was travelling southbound on Centre St directly behind MV#2 when he observed MV#2 sideswiped MV#1. The witness then observed the operator of MV#2 got out of his vehicle, looked at the damages to MV#1, retrieved a can of paint from a party standing on the sidewalk, got back into MV#2 and drove away. The witness stated MV#2 was a black colored "Dodge Ram" pick up truck bearing MA REG 2HLR81. The damages to MV#2 should be to its rear passenger side bumper. The witness described the operator of MV#2 as a white male in his 50's, 6' in height with a muscular build. A query of MV#2 showed that it is registered to a female party out of Weymouth MA. I attempted to contact the owner of MV#2 at phone #781 974-0307 and left a voicemail. I spoke to the Weymouth Police Dept and they will attempt to make contact with the owner of MV#2. I was able to speak to the owner of MV#2 and she stated her ex-boyfriend Richard Woodard has possession of MV#2 but does not speak to him often. She does not know Richard's personal information and provided me with his contact number of 617 631-0596. I spoke to Richard and he stated he was contacted by the owner of MV#2 and was already enroute to Police Headquarters. Richard arrived shortly after and he confirmed that he is the operator of MV#2 and was on Centre St retrieving paint earlier. Richard stated he was not aware he struck any other vehicles. I inspected MV#2 and did not observed any damages to its entire right side. I observed some minor scraping marks on the rear passenger side tire of MV#2. The rear passenger tire of MV#2 would be the most likely part of the vehicle that made contact with MV#1 and also consistent with the witness's statements. Based on the statements made to me by Richard and the fact that there were no visible damages to MV#2 besides its rear tire, it is possible that Richard was not aware he made contact with MV#1. All parties involved were provided with this accident report and advised to contact their insurance providers. There were no reported injuries to all parties involved. I will not pursue this matter further.

Massachusetts uniform citation #T2012863 was issued to the operator of MV2 for MGL Ch. 90 Sec. 10 Operating MV w/o a license and MGL Ch. 90 Sec. 13B Use of Electronic Device. No injuries were reported on scene.

Witness Kaitlyn Coker stated she was travelling on Beacon St W/B directly behind a white sedan bearing MA reg 3910ZR(#2). Coker stated this vehicle while appearing to pull over to the right in an attempt to park, struck an unoccupied parked MV MA reg 2BAK94 (#1) in front of 845 Beacon on the driver's side front. Brown of 114 Monroe St Dedham MA 02026. An attempt to find a phone listing for this vehicle was negative. I left a note on windshield of #1 with witness and other vehicle insurance company listing. #2 would have damage on the passenger side with grey paint from #1. Traffic Bureau Update (Officer Gaudet): On Friday, December 18, 2020 I spoke with the owner of MV2, Mark Brown (S38802538). Mr. Brown stated he was operating his 2016 Lexus ES (MA: 3910ZR) on Beacon Street at the time of the crash. Mr. Brown stated he was thinking of parking in the area. Mr. Brown stated when he was pulling into the spot he decided he didn(cid:31)t have enough time to do his shopping and left the area. Mr. Brown stated he did not know he was involved in crash. Mr. Brown stated when he got home he saw a scratch on his passenger side but did not call the police because he thought insurance would handle it. Mr. Brown will be mailed Massachusetts Uniform Citation T0115919 for Newton City Ordincance Chapter 19, Section 75 (Failure to Use Care). Beacon Street is a public way maintained by the City of Newton.

On Monday 2/22/2021 at approx 0849hrs, while assigned to N491, I responded to 33 Austin St in Newton for a MVA hit and run. There I was met by the owner of MV1 who states that his car was parked in the Austin St lot while getting coffee at Starbucks. He states that a witness to the accident gave him their information. Witness statement: Schreiner states that she had just left Starbucks and heard a crash. When she looked into the Austin St parking lot she could see MV2 make contact with MV1. She said that MV2 then scrapped the side of MV1 with the front passenger side of the vehicle. When MV2 left the parking lot she said that she was able to read the MA plate of 1NTE29. Schreiner said that MV2 was white and looked like a Toyota Highlander operated by a white female w/ blonde hair in her 40's. It is unclear at this time who was operating MV2(MA# 1NTE29). Dracut PD was contacted about the registered owner and I was able to get a phone number. However, the number was to a business she hasn't worked at in five years. Traffic Bureau update (Officer Gaudet): On Monday, February 22, 2021 I was able to make contact with the registered owner of MV2, Ms. Melinda Russo (S29162725). Ms. Russo stated that she was operating her white 2019 Toyota Highlander (MA: 1NTE29) in the Austin Street municipal parking lot this morning. Ms. Russo stated she was backing out of a parking spot when she observed her passenger side mirror collapse in. Ms. Russo stated after she exited the parking spot she pulled her vehicle over and observed no damage to the passenger door mirror area. Ms. Russo stated she fixed her mirror and then left the area. Ms. Russo stated she did not think there would be damage to any other vehicle because she did not have damage on her vehicle. Ms. Russo was advised that if she believes she was involved in a crash, she should make every effort to contact the owner of the other vehicle. Ms. Russo was also advised she can contact the police department for assistance. Ms. Russo will be mailled Massachusetts Uniform Citation T1446762 for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care). Austin Street is a public way maintained by the City of Newton.

Needham Officer went to Vehicle 2 residence to check damage on vehicle. Vehicle 2 owner would not allow Needham Officer into his garage to inspect damage.

On April 19th, 2021 at approximately 12:48 hours while working N491 I responded to #146 Norwood Ave for a report of a past MV crash. On my arrival I spoke with the R/P, owner of the vehicle involved, Ann William. She stated she received a call from her daughter around 11:40 this morning that someone had backed into her vehicle(2019 Subaru wagon) which was parked in the driveway to the above address. The other vehicle involved had driven away after the crash. Ms William informed me of a witness identified as Wagner Calhau (978-413-3927). She stated she had already spoken with him and he emailed her a copy of the video he took of the other involved vehicle. The witness apparently was doing work across the street when he witnessed this accident. I located the witness who was still on scene across the street. He reported the following: A truck was dumping some gravel to his job site and had temporarily blocked the street. A pick up truck heading W/B became impatient waiting for the truck to move and began to turn around to head back E/B. The pick up proceeded to back up into the driveway of #146 Norwood Ave to turn around. The truck as it backed into the driveway backed into the rear drivers side to vehicle #1 causing damage. The witness then grabbed his cell phone but was only able to begin recording as vehicle #2 was driving away. Witness showed me the video footage he captured on his cell phone. The pick up truck appeared to be silver in color with a tarp in the back. The license plate I couldn't discern. The operator was not visible.from the video. I will have the video emailed to our IT bureau to see if they can enhance the video for the purposes of capturing the license plate for the pick up truck. I will further investigate if IT bureau is able to enhance the video that shows the suspects license plate.

On 04/20/21 at 11:46 hours, I (Traffic Unit 523) responded to the CVS parking lot located at 978 Boylston Street for a city motor vehicle hit & run accident. The city vehicle was struck when the operator was inside the store and the other unknown vehicle left the scene. The CVS parking lot is a thru-way for the public to access and park for the store. The weather at the time was partly cloudy and the road conditions were dry. On arrival, I met the driver of the city vehicle identified as William Bertrand (MA D/L# S24973075). The vehicle is a 2016 Ford Escape color black with MA registration # M2534A and is labeled as vehicle number 304. Mr. Bertrand stated he was parked in the lot and went inside the store. The Escape was parked in the seventh spot in from the parking stalls closet to the entrance of the store. He then came out and observed that the vehicle had been struck on it's rear driver's side quarter panel area. He says the other vehicle had left the scene and no note was left on his vehicle. I observed numerous scrape marks to that mentioned area and I took digital photos of the damage. I also observed two small specks of paint on the damaged area that were a bluish gray type of color from the suspect vehicle. It appears that due to the scrape marks that the unknown vehicle was pulling in to the parking space to the left of the city vehicle. As it was pulling in, it struck the city vehicle with it's front passenger quarter panel area. I spoke to Nick (CVS manager) on the phone and was inquiring about possible surveillance footage of the lot. He stated that he didn't believe the cameras cover that area of the lot but he would look for any possible footage and notify me if so.

On Thursday May 13th, 2021 at approximately 1353 hours myself and several units responded 55 Chapel Street for a motor vehicle that struck a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. Chapel Street is a public way owned and maintained by the City of Newton. Upon arrival Fire and medical personnel were attempting to remove the pedestrian from underneath the vehicle as her right leg was stuck in the undercarriage. Fire and medics were able to successfully free the pedestrian from the vehicle and render medical attention. The pedestrian was identified as Beth Herr ( S58524067) and it was determined that she was also the owner of the vehicle involved. I spoke to witness Joe Shields who stated he was outside his house located at 56 Chapel Street when his attention was brought to a blue SUV that was unoccupied moving backwards down Chapel Street. Joe stated he first witnessed the operator of the motor vehicle pull up to the UPS drop box located on the sidewalk across the street from his house. He stated he saw the female operator step out of the vehicle and drop off a package. Next he saw the vehicle start to roll backwards and the female operator opened the drivers side door and attempted to pull the emergency break. He stated the vehicle overtook her and she fell to the ground with her foot stuck under the car. Joe states he heard her yell and saw her get dragged by the vehicle to the opposite side of the street where the vehicle was stopped by the curb. Joe stated he tried to assist the woman but she was yelling in pain and he did not want to cause more injury so he waited for help to arrive. Witness Robyn Preston stated she was walking on Chapel Street when she saw the blue SUV rolling backwards with no one inside the vehicle. She states she then saw a woman try to stop the vehicle but she got dragged underneath the vehicle to its final resting place. Robyn stated she stayed on scene until help arrived. I was able to speak with Beth inside the ambulance prior to being transported to the hospital. Beth was conscious and alert but in a considerable amount of pain and discomfort. I asked her what happened to which she stated she drove her blue Toyota Rav4 MA REG: 1FHM53 to the UPS drop box located in the area of 55 Chapel Street. She stated she thought she put the vehicle in park but must not have as it started to roll backwards when she got out of the car. She states she tried to stop the car by opening the drivers side door and attempting to pull the emergency break. The car over took her and she was dragged to the ground with her right foot stuck underneath the vehicle. She was dragged across the street where the vehicle was finally stopped by a curb. Beth sustained minor scrapes and road rash on various parts of her body and her right leg was injured. Fallon medics transported her to the Brigham & Womens Hospital to be treated. Her husband was contacted using Beth's cell phone and notified of the accident. Captain Boudreau and I were able to drive her vehicle to her residence at 50 Broadway Street, Newton. Her vehicle was left parked outside her house and her car keys were left with her husband who was inside the residence. The vehicle did not sustain any damage. Pictures of the scene were taken and submitted to the IT Bureau. I will be following up with Beth at the hospital to check on her status. On Friday May 14th at 0900 hours I spoke with Beth on the phone. She stated she is home from the hospital and doing much better. She states she has right leg pain and road rash from the accident.

On Wednesday, May 19th 2021, at approximately 4:40pm, I, Officer Brooks, responded to 457 Walnut street, the Newton North High school, for a hit and run that occurred in the parking lot. I spoke with the operator of MV1(MA REG 672MT6), who stated his vehicle had been parked in the lot all day. When he came out he found a note on the car saying sorry that I backed into your car, and a phone number (617-899-4204). No other information was provided. MV1 had a plastic fascia piece broken off of the front bumper. Attempts to reach the other driver at the number provided were unsuccessful and a voicemail was left. The operator of MV1 was provided with the report number. There are cameras on the high school that face the parking lot and may have caught the accident, but I was not able to access them at this time. MV1 was parked in the 4th row of spots if you count them from the wall of the school out towards the street. Traffic Bureau update (Officer Gaudet): I was able to obtain a name associated with the phone number provided on the note of MV1. The phone number is registered to Mr. Dominic Matos (SA2100551). I attmepted to contact Mr. Matos with a negative result. I left a message for him to contact me to provide a statement.

MV1 owner states they were alerted by the bouncer at 107R Union St that their vehicle had been hit. Bouncer, Steven Raveri, states he witnessed a BMW back into MV1 in the alley behind Thistle and Leek and then leave the scene. Another worker from Thistle and Leek, Christian McNeill, states he witnessed a BMW back into the Subaru more than once attempting to turn around before leaving the alley. I observed paint damage on the rear end of the Subaru. Steven was able to take a picture of the BMWs plate (NY: MD60330) before it left. I was able to find the vehicles information and two phone numbers for the registered owner. I will be contacting them on my next tour of duty. Traffic Bureau update (Officer Gaudet): On Thursday, May 27, 2021, I was able to make contact with the registered owner of MV2. Mr. Stephen Gorfine (NY: 392 858 797) stated he was operating his 2007 BMW 750 (NY: 60330) at the time and location of the crash. Mr. Gorfine stated he was not aware he was involved in a crash. I explained to Mr. Gorfine that two witnesses observed his vehicle crash into MV1 as it was parked unoccupied. Mr. Gorfine stated he has hit many cars in the past and has been hit himself. Mr. Gorfine did not appear to understand the brevity of causing damage to MV1 and not stopping to attempt to provide his information to the owner. Mr. Gorfine was advised and mailled Massachusetts Uniform Citaton 019187AB for Newton City Ordinance Chapter 19, Section 75 (Failure To Use Care).

On Monday, June 28, 2021, while assigned to Traffic unit N525, I was entering my marked cruiser to leave the parking lot at the rear of 1321 Washington Street, Newton. The rear parking lot of the police station features a public right of access, but is restricted for authorized vehicle. There are signs posted at both entrances to the parking lot stating, "RESTRICTED AREA Newton Police Department Authorized Vehicles Only Beyond This Point. As I entered my cruiser, I observed a red 2020 Jeep Wrangler (CT: AX89516) parked directly across from my cruiser facing Eastbound. This vehicle was not parked in a marked parking spot and was the last vehicle in the parking lot from the Cherry Street entrance on the North side of the lot. I observed that the front bumper of this vehicle made contact with the rear bumper area of a parked 2018 Tesla Model 3 (EV3617 ). I exited my vehicle and walked over to the vehicles to investigate if a crash had occurred. I observed a black male party sitting in the front passenger seat of the Jeep. The driver seat was vacant. The Jeep had its driver door and passenger door windows rolled down. The vehicle was not running and there were no keys in the ignition. As I approached the passenger side door, the male passenger appeared to be asleep and I could detect a moderate odor of alcoholic beverage emanating from the passenger seat area. I woke up the passenger and inquired where the driver of the vehicle was. The passenger identified himself by his Connecticut ID as Masengo Kabongo (#178372788). Mr. Kabongo stated his cousin Marcel was the operator of the vehicle and they had pulled into their current parking spot so Marcel could run an errand. Mr. Kabongo stated he did not know where his cousin was, and he could not locate his phone to contact his cousin. I asked Mr. Kabongo if he was aware he was parked in the Newton Police Station parking lot and that the vehicle he was sitting in crashed into an employee's vehicle parked in front of them. Mr. Kabongo stated he was not aware. Mr. Kabongo appeared to be very confused with the situation and continued to state he did not know why they were there and where his cousin was. The registered owner of the Tesla arrived and assessed the damage to his vehicle. I observed minor damage to the rear bumper area to MV2. There was also minor damage to the front bumper of MV1. The owner of MV2 was able to review footage of the crash from his vehicles exterior surveillance system. While viewing this video with him, I observed MV1 enter the Newton Police Department parking lot from Cherry Street and pull right behind MV2. MV1 appeared to be occupied by two black males at this time. One in the passenger seat and one in the driver seat. MV1's front bumper area is then observed crashing into the rear bumper area of MV2. The Newton Police Department IT Bureau was able to observe the crash from a surveillance camera affixed to the Newton Police Department maintenance garage (facing Cherry Street). The video captures MV1 entering the parking lot and crashing into the rear of MV2. A black male with light colored shorts and a dark T-shirt is observed exiting the driver side door and walking away from the area up Cherry Street (S) to Washington Street. I explained to Mr. Kabongo that the crash was captured on our surveillance system and that his cousin appeared to be the operator. Mr. Kabongo stated his cousin was currently in Newton District Court being arranged for a previous incident (Report #21027283). I looked up recent bookings for NPD and I observed a party matching the name Mr. Kabongo provided. On Friday, June 25, 2021. Mr. Marcel Mengue-Ngoumou was arrested and cited by the Newton Police Department for OUI (Subsequent Offense), Negligent Operation, Marked Lanes, Open Container, and Unlicensed Operation. I contacted the Newton Police Department Court Prosecutor, Officer Hagopian, to see of Mr. Mengue-Ngoumou was still present in Newton District Court. Officer Hagopian stated he contacted the court and Mr. Mengue-Ngoumou was waiting to be arraigned. Myself and Officer Hagopian responded to Newton District Court to try and speak with Mr. Mengue-Ngoumou. Upon entering Session 1 in NDC, the lone individual sitting in the gallery was a party matching the description of the party from the surveillance footage. I spoke with Assistant District Attorney Nathan Stetson who was present in the court room. I apprised him of the current incident that Mr. Mengue-Ngoumou was involved in and told him I was going to speak with him. Mr. Mengue-Ngoumou at this time was sitting on the second to last bench on the left side of Session 1 as you walk into the courtroom. The exit to the courtroom was behind him. I approached Mr. Mengue-Ngoumou from the front of the courtroom and explained to him that I had just spoken to his cousin in the parking lot of the Newton Police Department. I explained to him the vehicle his cousin was sitting in had crashed into a parked vehicle belonging to a Newton Police Department employee. I explained to Mr. Mengue-Ngoumou that his cousin had identified him as the operator of the vehicle. I then asked Mr. Mengue-Ngoumou who was operating the vehicle when they were traveling to court today. Mr. Mengue-Ngoumou stated he was the operator. I asked Mr. Mengue-Ngoumou if he knew he had parked in the Newton Police Department parking lot. Mr. Mengue-Ngoumou stated he did not. I asked Mr. Mengue-Ngoumou if he knew he crashed into a parked vehicle in the parking lot. Mr. Mengue-Ngoumou stated he did not. I asked Mr. Mengue-Ngoumou if he knew he was not supposed to be operating a vehicle because of his incident from the other night and his license being suspended. Mr. Mengue-Ngoumou stated he did. I apprised ADA Stetson to my conversation with Mr. Mengue-Ngoumou. Mr. Mengue-Ngoumou was issued Massachusetts Uniform Citation 054528AB in hand for Chapter 90, Section 23(K) (Operating a MV after suspension, subsequent offense), and Chapter 90, Section 24 (Leaving the Scene of Property Damage). Mr. Mengue-Ngoumou knowingly crashed into MV2, parked his vehicle, took the keys from the ignition, and left the area with out making an effort to contact the owner of MV2. The 2020 Jeep Wrangler from the crash was removed from the Newton Police Department parking lot by Tody's towing. A towed MV form was completed and submitted. Washington Street and Cherry Street are both public ways maintained by the City of Newton.

On Tuesday 6/29/21 AT 6:45 PM Sean Ross Mcclinchy came into the station to report a Hit and Run. He stated that he was sitting in his car a green Ford Mustang (MV1 MA REG 6SR718) on Chapel street when a white Toyota Camry (MV 2 MA REG 9AP511) side swiped his car, they exchanged words and then MV 2 proceeded to leave the scene. MV 1 followed MV 2 and MV 2 eventually stoped in a parking lot in Watertown. The operator of MV 2 refused to give his information to the operator of MV 1 and drove away in his car for the second time. The operator of MV 1 was able to take down the license plate of MV 2. A few hours after the operator of MV 1 came in to report the Hit and Run I spoke on the phone with the operator of MV 2 and he stated that it was a tight squeeze on the street and he side swiped the MV 1. He also stated that he panicked and that is why he left the scene without providing his information. MV 1 was damaged on the drivers side with scratches on the door, the wheel well, and the mirror was broken. MV 2 had scratches on the drivers side also according to the driver of MV 1. Neither party was injured during this collision. I sent a Criminal Application in the mail to the operator of MV2.

On Monday, July 12th 2021, at approximately 5:24pm, I, Officer Brooks, responded to the parking lot of the Barn, 229 Walnut street, for a car accident. I spoke with the owner of MV1(MA REG 9937PH), who stated she had been parked here all day, and when she came out to her car there was a note on the windshield stating "Sorry i hit your car, foot slipped on wet". Her vehicle sustained damage to the drivers side front bumper. The note contained the license plate for MV2 (MA REG 54542), and the registered owners name. Dispatch attempted to reach her by phone but was unable to speak with her. MV2 was still on scene and parked in front of MV1. MV2 had minor damage to the front bumper. It appears the operator of MV2 was pulling into the parking spot and struck MV1. There were no injuries and both vehicles were able to be driven from the scene. The report number was given to both involved parties.

On 07/27/2021, while assigned to N494, I, Officer Conary, spoke with DEALE, Cathy (Operator of MV1) over the phone about a past hit and run. DEALE explained to me that she parked Westbound on Washington Street in front of 513 by the fire hydrant. Between 1200-1400 MV1 was hit on the driver side, and the mirror broke off. Operator of MV1 did not see any other vehicle standing by, she left the area and notified Newton Police. Operator of MV1 sent me pictures to confirm the damage. I went back to the area of where the accident was reported and confirmed glass and a pieces of a mirror on the side of the road.

Operator of vehicle 1 was backing out of a parking space in front of 199 Boylston St when he struck vehicle 2 which was traveling eastbound in the parking lot. Vehicle 2 did not stop and left the area. The plate given for vehicle 2 was 962WNZ and I had two phone numbers for the vehicle owner (h:617-558-7264 and C:617-834-7806). I made several unsuccessful attempts to contact the owner of vehicle two and will follow up on my next tour of duty. On 08/23/2021 I spoke to Julia over the phone who stated she was operating the vehicle and did not realize she had been involved in an accident until she got home. While driving through the parking lot she did notice the other vehicle was clost to her but did not realize they had made contact.

On Sunday, August 22, 2021, I was contacted by Sgt. M. Wade of the Newton Police Traffic Bureau at 0032 hours. Sgt. Wade stated a serious motor vehicle crash occurred on Washington Street in Newton and it involved a City of Newton marked police cruiser (N494). Sgt. Wade stated Officer Katelyn Pohlman and the operator of the other vehicle involved were transported to the hospital with potential serious injuries. I was requested by Sgt. Wade to respond to the scene from my residence and investigate the crash further. On arrival at the scene, I observed the two vehicles involved in the crash to be a 2015 Ford Explorer (MA: 586RT7) and Newton Police marked unit N494 (MA MVN: MP494B). These vehicles were observed disabled on the roadway in front of 612 Washington Street, Newton. There was a large debris field comprised of various parts from both vehicles on the roadway and in the parking lot of 612 Washington Street. The road surface at the time of my arrival was wet and there was light rain falling. Sgt. Wade reported that at the time of the crash the road was completely dry and it had not started to rain yet. Washington Street is a public way maintained by the City of Newton. The portion of Washington Street where the crash took place supports two- way travel, Eastbound and Westbound. There are two travel lanes in both directions. Travel direction is divided by a double yellow line in the middle of the roadway. The area is well lit and features numerous light poles illuminating the roadway. The closest intersection to this location is Crafts Street just west of the crash location. MV1 (MA: 586RT7) had substantial front-end damage, especially to its front passenger side and was inoperable. I observed this vehicle at its final rest on the roadway with its front end facing Northbound on Washington Street just over the double yellow line in the middle of the roadway. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV1 was removed from the roadway by Tody's Towing. MV2 (MA MVN: MP494B) had substantial front end damage and was inoperable. The impact from the collision caused the vehicle to spin uncontrolled and the final rest, with the front end of the vehicle facing the front entrance to the 7-11 (S). The vehicle's final rest was on the sidewalk in front of 612 Washington Street. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV2 was removed from the roadway by Tody's Towing. The Officer involved in the crash is Officer Katelyn Pohlman. Officer Pohlman stated she was operating Newton Police marked unit N494 on Washington Street (E) past Crafts Street. Officer Pohlman stated as she was operating her vehicle past 612 Washington Street (7-11), a vehicle in the westbound lane of Washington Street abruptly crossed the double yellow line in the roadway and entered her travel lane head-on. Officer Pohlman stated she did not have an opportunity to react to the vehicle in front of her and the front passenger side of MV1 crashed into the front end of her vehicle (MV2). Officer Pohlman sustained injuries to her left forehead area and both of her arms. Officer Pohlman was later transported to Brigham and Women's Hospital for evaluation and was released a short time later. It was reported to me by numerous Officers on scene that Officer Pohlman exited her cruiser after the crash and rather than tend to her own injuries, she focused her efforts on rendering aid to the operator of the other vehicle involved. Officer Pohlman found the operator of the other vehicle lying on the floor of the front passenger seat under the dash. Officer Pohlman rendered aid to the operator while waiting for Newton Medics to arrive. Officer Pohlman was also assisted in rendering aid to the other operator by Officer Conary and Officer Larson. Officer Larson reported that while interacting with the operator of MV1, the operator repeated numerous times, "Is she ok? Is she ok?". Officer Larson also stated the operator said to him "I feel bad because I was texting on my phone". Officer Larson stated the operator of MV1 works for the Department of Corrections Special Operations Unit and has Department issues equipment inside the vehicle. The following items were placed into Support Services, locker 9 for safekeeping by Officer Larson: 1 Gray Pelican Gun Case, 1 Black Gun Belt, 1 Taser, 1 Can OC Spray, 15 (50) Round Cases Of Ammunition. The operator of MV1 (MA: 586RT7) was identified as Mr. David Cafarelli (S16965940). Mr. Cafarelli was transported to Newton Wellesley Hospital for a potential head injury before my arrival. I spoke with Mr. Cafarelli at Newton Wellesley Hospital in the Emergency Room with Sgt. Wade. Mr. Cafarelli's mother and father were present when we spoke with him. Mr. Cafarelli stated he was out with friends before the crash and was driving to the 7-11 on Washington Street. Mr. Cafarelli admitted that before the crash, he had his cell phone in his hand while he was operating his vehicle. Mr. Cafarelli stated he does not remember if he was texting or changing the music on his phone at the time of the crash. Mr. Cafarelli stated he looked up and was unable to avoid the vehicle (N494) directly in front of him. Mr. Cafarelli stated he also does not believe he applied his brakes before the collision. When we spoke with Mr. Cafarelli in the Emergency Room, he was still waiting for test results to determine the severity of his injuries. On Monday, August 23, 2021, Sgt. Wade responded to 612 Washington Street, Newton to retrieve surveillance footage from the 7-11 of the crash. 7-11 stated their outside surveillance system was not operating correctly, but they were able to capture some of the incident from a camera inside the store. I observed this footage. MV1 is observed traveling on Washington Street (W) in the westbound travel lane in front of the storefront. MV1's driver side is then observed traveling over the double yellow line in the middle of the roadway and into the eastbound lane of Washington Street. A head on collision ensues and MV1 can be seen rotating counterclockwise in the roadway backwards to its final rest location on Washington Street. MV2 is then observed at its final rest position on the sidewalk facing southbound in front of the 7-11 storefront. After completing my investigation, I have surmised the following. Mr. Cafarelli was operating his 2015 Ford Explorer (MA: 586RT7) in the area of 612 Washington Street, Newton. Mr. Cafarelli was holding his cell phone in his hand while operating his vehicle. Mr. Cafarelli became distracted while he was operating his vehicle due to his attention being focused on his cell phone. Mr. Cafarelli traveled over the double yellow line from the westbound lane of Washington Street and entered the eastbound lane of Washington Street. Newton Police marked unit N494 was traveling on Washington Street (E) in the area of 612 Washington Street at this time. Mr. Cafarelli's vehicle crashed into the front end of Newton Police marked unit N494. Mr. Cafarelli was not wearing a seatbelt and as a result of the impact from the crash, he was ejected from the driver seat and his final rest inside the vehicle was in the flood of the front passenger seat. As a result of my investigation, Mr. Cafarelli was issued Massachusetts Uniform Citation for Chapter 90, Section 13B (Hands-Free Violation), Chapter 89, Section 4a (Marked Lanes Violation), and Chapter 90, Section 13a (Seat Belt Violation). Photos from the crash scene and the surveillance media from 7-11 capturing the crash were submitted to the IT Bureau.

Upon arrival, I observed vehicle 1 parked on Richards Circle with minor damage to the drivers side mirror. Operator 1, Matthew Kelly, stated he was parked by the curb on Crafts St (public way in the City of Newton ) across from F.A. Day Middle School and when he entered his vehicle and closed the driver's side door, vehicle 2 side swiped his vehicle causing damage to his mirror. He then saw Massachusetts Registration: 1GRH91, a White 2012 Nissan Rouge, continue northbound on Crafts St and not stop. Operator 1 stated he followed vehicle 2 on Crafts St until he got to the Waltham line on Waltham St. Operator 1 then pulled into Richards Circle and called dispatch to report the accident. Operator 1 stated he could not tell if Operator 2 was a male or female, but stated there was damage to the passenger side mirror of vehicle 2. The registration for vehicle 2, comes back to a Walter and Analise Wilson out of Waltham. Waltham PD responded to both of their addresses to attempt to raise them with negative results. There was also not a phone number listed for either party. There was no injuries reported and no tows were needed. Traffic Bureau update (Officer Gaudet): I attempted to contact both registered owners for MV2 with a negative result. A "Hit and Run" inquiry was mailed to Mr. Walter Wilson and Ms. Analise Wilson regarding their possible involvement in this crash.

On September 2, 2021, I responded to the front desk of Newton Police Headquarters to speak with Charles Rankin who wanted to report a past hit and run. Mr. Rankin stated his vehicle (vehicle 1) was parked unoccupied by the curb at 70 Orchard Ave ( Public way in the City of Newton) on Tuesday, August 31. Later that day, he discovered damage to his drivers side mirror. He stated that a note was left behind stating that Massachusetts registration: V91699 struck his vehicle and left the area. He does not know who left the note due to no contact information being left behind. Mr. Rankin showed me a picture of the damage and it appears vehicle 2 side swiped vehicle 1 causing significant damage to the mirror. Mr. Rankin did research and discovered that the plate is registered to JCAC Enterprise out of Milford and provided me with a phone number. It should be noted that when I queried the plate it comes back to that company as well. I will attempt to follow up with the company during business hours on my next tour of duty. On September 3, I called JCAC Enterprise and spoke with an employee who stated they do not have any information on someone driving a truck in the area. They also stated that Mr. Rankin called them and they are going to pay for damages.

Mv#1 was parked unoccupied opposite #8 Burrage Rd (N/B). #1 was sideswiped by an unknown Mv that left white paint transfer damage on #1's driver's side front end side and also struck the driver's side mirror all causing moderate damage. I spoke to the resident at 8 Burrage Rd. The resident stated a visitor in his house, Jonathan Kaplan, witnessed the accident. The resident stated Kaplan told him he observed a white sedan bearing MA Reg CIAF31 (Mv#2) attempting to parallel park between #1 and another Mv. During the attempt, #2 was observed sideswiping #1. Kaplan further stated a male operator got out of #2, left a note on #1 and drove away. Mv#1 owner/operator Ms Joan Beer stated no note was ever found. Later from HQ, I called the witness phone number provided (617-455-2557) by the resident of 8 Burrage Rd. I called that number, received VM and left a message. A query of #2 registration comes back on a '18 white Lexus G350 to a Mr Barry Benjamin of 79 Florence St #510S Newton. I received information Mr Benjamin's phone number was 339-223-0079. This number when called was not Mr Benjamin's. Later from HQ, I spoke to Ms Beer and provided her with the witness plate number observed as the paint color is a match to the damage sustained on her Mv. She was advised insurance company will investigate further based on current information received. Later, the witness Mr Kaplan returned my phone call and provided the following statement: Kaplan stated he was in his driveway outside directly across the street from where the accident was taking place. Kaplan stated the accident was very obvious as damage was unfolding while Mr Benjamin was attempting to squeeze between Mv#1 and another vehicle. Kaplan stated after realizing he (Mr Benjamin) could not fit he started to drive away. Kaplan stated it drew the attention of another witness (unknown) standing on the street when both walked up to Mr Benjamin's vehicle and asked him if he was going to leave a note for the damaged vehicles. Benjamin refused. Kaplan described Benjamin's overall demeanor as "rude." Kaplan stated he told Benjamin he has cameras capturing the incident. Not until then, Benjamin got out of his car and left a note on only the other vehicle reported to be minor license plate damage. Kaplan stated Benjamin never left a note on Mv#1, the vehicle with obvious moderate damage. Kaplan stated he actually viewed the note left on the other vehicle and said it was illegible. Kaplan told Benjamin this and Benjamin yelled to him "mind your own business" and drove away. Kaplan stated his ring doorbell camera was not positioned to capture any of the incident. Traffic Bureau update (Officer Gaudet): On September 8, 2021 I was able to make contact with the operator of MV2, Mr. Barry Benjamin (S42076881). Mr. Benjamin stated he was operating his 2018 Lexus G350 (MA: CIAF31) at the time of the crash. Mr. Benjamin stated he was backing his vehicle into a parking spot and the space ended up being too small. Mr. Benjamin stated the rear of his vehicle crashed into the front bumper area of MV1, knocking off it's license plate. Mr. Benjamin stated he got out of his vehicle and put the plate back on MV1. Mr. Benjamin stated he left a note in the vehicle with his contact information and left the area. I asked Mr. Benjamin if he was aware of making contact with another vehicle in the area. Mr. Benjamin stated he did not make contact with any other vehicle. Mr. Benjamin stated there was no damage to his vehicle and stated he would e-mail me pictures of his vehicle and pictures of MV1 that he took on scene. Those pictures were forwarded to the IT Bureau to be added to this report. Mr. Benjamin also stated in his e-mail, "My wife informed me that the homeowner who contacted the police called my wifes attention to a car up the street. The car was on the opposite side near a dirt road that had been side swiped earlier. My wife said, "That has nothing to do with us" and the homeowner agreed that that situation did not involve our car." At this time, no charges will be filed against Mr. Benjamin for leaving the scene of property damage.

On 9/7/21, Brooke Movitz DOB 5/6/99 came to NPD Headquarters to report a hit & run of MA reg 646NM7. Movitz states she was parked in the Newton Centre Lot in a row of parking stalls near Walgreens on the Langley Rd side. Around 1315 hrs, Movitz noticed damage to the left rear side of her vehicle. She stated that the vehicle parked next to her, MA reg 298MS7, had damage to the front left side and was parked at an angle that may suggest an improper turn into the parking spot (see diagram). Movitz was able to show me two pictures of minor damage to both vehicles, however I am not able to confirm that the damage is related to each other via the photographs. A white male with spiky hair in his 40's drove off in the vehicle, according to Movitz, but there was no interaction between the two. I was able to make contact with the owner of the vehicle in question, who told me that his son Daniel Suleski III DOB 4/10/92 was operating his car today. Suleski was able to be reached by phone and was adament that he did not hit another vehicle. Suleski did state he was parked in the Newton Centre Lot around the time in question. Officer Marini checked the area for any cameras that may record the area with negative results. Based on the totality of the facts presented, I am unable to reach a definitive conclusion at this time. Movitz was advised to contact her insurance company for further assistance.

I spoke to the homeowner of 15 Walsingham St identified as Joonho Choi who stated to me the following: On or about 8:48 AM this morning a Jeep Grand Cherokee was turned into his driveway in order to turn around on Walsingham St and in the process, the MV struck his stone wall causing moderate damage. The driver of the MV identified himself to the homeowner and they exchanged paperwork. I advised Choi I would document the crash for him. There is unknown damage to the MV due to the fact that the driver had already left prior to my arrival and no phone number was provided. Choi has the accident recorded on his surveillance cameras and was advised to keep all pictures and videos for insurance purposes. The driver of the MV is identified as Bruce Roberts (4/2/55).

There was minor paint transfer to the rear passenger side quarter panel. There was a note left on her car by two witnesses stating they saw a vehicle hit the car. I responded to the home of the other alleged driver involved and was unable to get a statement from this person. On 9/20 I responded to the home once again and was unable to make contact. I left a note on the vehicle saying to call NPD with my name attached. I also found a phone number to this party in the census, I called and left a voice mail. To be further investigated. 9/22 update, Emma Tsypina has not responded to the numerous attempts that have been made to speak with her about this incident.

On 09/15/21 at approximately 08:00 I responded to the intersection of the Rt. 9 East off ramp and Chestnut St for a report of a two vehicle crash. Upon arrival I found the operator of veh #1 pulled into the driveway of #742 Chestnut St. The operator of veh #1 stated she was driving South on Chestnut St when veh #2 drove from her right off the exit ramp without stopping and struck her in the passenger side front. Operator #1 stated operator #2 gave her a license but she never got the license plate or insurance company. On the above date and time while on Patrol in N496 I responded to 314 Hammond St (Boston College) to assist BCPD with damage to a wall. On arrivial I spoke to BCPD who stated they were alerted by an employee (Luckny Capre) that NP landscaping pulled onto the property to turn around and while pulling back onto Hammond St struck the stone wall ripping it apart. While speaking to BCPD thier dispatch was able to locate the landscaper by phone who stated they were in Newton Corner area and would come back. The landscaper returned and was identified as Luis Chingo-Calel and stated he knew he struck the wall but did not know what to do. On 9/20/21, Nipa Goud DOB 4/19/77 came to NPD Headquarters to report a past hit & run of MA reg 5RR913 ( vehicle #1). Goud states she was shopping in Johnson String 1029 Chestnut St on 9/15 around 1300 hrs when a good samaritan came into the store. He stated that the above mentioned vehicle had been hit in the parking lot. The witness is Allyn McCourt (617) 875-3457. He provided Goud with a description of an older female driving MA reg 2EE316. It was reported that she made contact with the rear of vehicle #1 and immediately left the scene. I spoke to Mccourt on the phone, who informed me that the woman was a white female between 60-70 yrs old. She appeared to be making a 3-point turn in the lot, hit a bush, then the rear bumper of vehicle #1. I asked Mccourt if there was any indication that the woman knew she hit vehicle #1, to which he stated "it certainly seemed so, because she motioned for me to move out of her way after she hit it". The vehicle was described by Mccourt as a blue sedan. MA reg 2EE316 comes back active on a 2011 blue Toyota Camry to Sergey Klepikov out of 1156 Chestnut St in Newton. QED shows a Vera Klepikov residing at that same address with a DOB of 12/22/52. I have left a message with the number on file for the Klepikov residence for Vera to contact me. Damage to vehicle #1 appears to be minor-moderate scraping from the picture I was shown by Goud. To be further investigated.

The witness stated he was inside 821 Beacon St the "Stretchmed" store at 11:55 when he observed MV#1 backed up into 2 mail boxes dislodging both from its foundation (1 blue colored and 1 green colored mailboxes) . The witness stated MV#1 stopped then proceeded to drive away. The witness did not get any registration information but stated that MV#1 was a white colored commercial box truck and was some sort of vegetable company. 821 Beacon St is directly next to 827 Beacon St the "Sandwich Works" store. I spoke to the owner of Sandwich Works and she confirmed she received food deliveries around the same time the accident occurred. The delivery commercial truck is from "Boston Salads & Provisions" with contact phone number of 617 307-6340 (57-61 Foodmart Rd in Boston MA). I spoke to Boston Salads & Provisions manager "Matt" who confirmed that his driver did make deliveries to Sandwich Works at 11:55. Matt stated his driver's name is a Carlos Lopez (Phone #781 885-8404). I spoke to Carlos and he confirmed he made a delivery to Sandwich Works. Carlos stated a vehicle had just left a parking spot in front of him so he pulled into that spot to park. Carlos stated he was unaware he had struck anything. Carlos stated there were no damages to the entire right side of his vehicle besides old damages. Carlos provided me with his driver's license and MV#1's registration (Apportioned REG 2745B). He was forthcoming with his information and was not evasive. Newton Dispatch notified Postal Police regarding the 2 mailboxes.

On October 4th, 2021 at approximately 11:58 hours while working N491 I responded to a report of a past Hit & Run involving a parked MV in front #350 Watertown St, Colonial drug. On my arrival I located one of the involved vehicles a 2009 blue Toyota Venza MA plate RUBRIC with the owner, Suzanne McGowan inside it. She reported parking her car in front of #350 Watertown St while out doing errands. She came back a short time later and found her vehicle had been struck. She found a note attached to her vehicle that said," I'm sorry I hit your car, with a telephone # 617-448-1039," call me". I spoke with the R/P, witness JP Botindari, address Colonial drug, #350 Watertown St. He reported hearing a crash from inside his business which brought him outside to investigate. He then observed a blond haired white female, 40's ruddy complexion wearing ripped blue jeans put a note on the parked vehicle she just hit. He asked her if she was ok? Women said nothing before getting into a grey colored SUV Ma reg.888-oyd.with heavy passenger side damage. The vehicle fled E/B on Watertown St. The registration was later queried but didn't come back to any vehicle. The phone # left on the note was traced back to a women out of Arlington Ma. but after dispatch had spoke with her realized this was not the women involved. The only vehicle attached to her name was a Nissan Pathfinder. Her drivers license picture didn't match the witnessed description either. An attempt will be made to identify the other vehicle through similar license plates. Traffic Bureau update (Officer Gaudet): A query of MV2 (MA: 8880YD) showed it comes back to a 2016 gray Jeep Cherokee registered to Ms. Johanna McGlame out of Newton. I spoke with Ms. McGlame via telephone. Ms. McGlame stated he was involved in the crash and did leave a note on the vehicle she hit with her phone number. Ms. McGlame stated she was traveling on Watertown Street and the passenger side of her vehicle made contact with a parked car. Ms. McGlame stated her vehicle had damage as a result of the crash and it is currently being fixed. Ms. McGlame's vehicle information and contact information was updated in this report. Ms. McGlame will not be cited for leaving the scene of property damage.
