On the above date/time I responded to Lexington Street / Freeman Street for a two motor vehicle crash with air bag deployment. Upon arrival on scene I checked all parties for injuries. Operator of vehicle one Ms. Farnese complained of head / neck injuries and Fire and Medics responded on scene. All parties in vehicle two did not have any injuries. I spoke with the operator of vehicle one Ms. Farnese in the ambulance she stated she was slowing traveling south on Lexington Street at the intersection of Freeman Street due to the traffic signal for Lexington Street was yellow. She stated as she was slowing she felt a heavy impact from in the rear by vehicle two. I spoke to the operator of two Ms. Heimbrock, she stated she is a pastor for the Salvation Army in New York and she was transporting two children Shamah Lapaix d.o.b. 4-18-14 and Grace Lapaix d.o.b. 1-10-11 to a Christian school in Wayland. She stated that she is staying with the children at 65 Falmouth Road Newton MA with the childrens grandmother due to the fact the parents are in New York doing a continuing education program until Friday January 18, 2019. She stated she is not familiar with the area, she was traveling south on Lexington Street approaching Freeman Street when she glanced at the map on her cell phone. She stated she did not observe the color of the traffic signal and did not observe the rear brake lights in vehicle one. All parties were evaluated in vehilce two by medics with no injury. I spoke with the childrens mother Sarah Lapaix 1-203-685-2809 who confirmed Ms. Heimbrock's statement about the care of the children. I did observe an expired inspection sticker as of 12/18 on vehicle one. Therefore due to operator of vehicle two's statement I checked the rear brake lights. When the brake pedal was pressed the left brake light and top(3rd) brake light was operational but the right rear brake light was not operational. Tody's towing arrived on scene and towed both vehicles top their lot. Ms. Heimbrock made arrangements for transport of herself and the children. Ms. Farnese was transported to Newton Wellesley Hospital for non life threatening injuries. I advised all parties of the process and gave them each others information. I checked the traffic signals in the intersection and they were fully functional. Therefore with all the above mentioned I am issuing citations to both parties Operator of vehicle one citation # T1440538 for Chapter 90 Section 20 EXPIRED INSPECTION STICKER $ 55.00 Operator of vehicle two citation # T1440539 for City Ordinance 19-75 FAILURE TO USE CARE IN STOPPING $ 20.00 Both citations were filled out after investigation and will be mailed to their license address.

ON 1/18/2019 AT APPROX 0929HRS WHILE ASSIGNED TO 497 i RESPONDED TO THE AREA OF 1830 BEACON ST FOR A REPORT OF A TWO CAR COLLISION WITHOUT INJURY. I SPOKE WITH THE OPERATOR OF MA DEALER REG 5550 MARISSA DEVLIN WHO STATED SHE HAD STOPPED FOR A PEDESTRIAN IN THE CROSSWALK WHEN SHE WAS STRUCK FROM BEHIND. I THAN SPOKE WITH THE OPERATOR OF MA PASSENGER REG 3NM859, THOMAS DORRANCE WHO STATED ' IT APPEARS I'M AT FAULT' DORRANCE STATED HE HAD NOT NOTICED DEVLIN SLOWING AND STOPPING AND RAN INTO THE BACK OF HER. DEVLIN WAS ALSO COMPLAINING OF PAIN FROM THE STEERING WHEEL AND AIRBAG, CATALDO RESPONDED. NFD AND TODYS TOW ARRIVED ON SCENE .NFD USED THE JAWS TO SEPARATE THE TWO VEHICLES TODYS WAS ABLE TO TOW BOTH VEHICLES. DORRANCE SIGNED A PATIENT REFUSAL WITH CATALDO. NFD SPREAD SPEEDY DRY AND CLEARED THE ROADWAY.

On 2/12/2018 at approx 1445hrs while assigned to 497 I responded to the area of 659 Chestnut St for a report of a two car crash . Upon arrival Ma Reg 9VA441 being operated by Steven HUTCHINSON and Ma Reg 98NL90 being operated by Vi DANG were pulled to the side of the road, there was a heavy snowfall and the road had accumulated a thick layer of snow and multiple crashes were occurring. NFD was on scene , DANG was crying and hysterical despite the minor nature of the crash and zero damage to either vehicle. HUTCHINSON stated they were creeping along NB on Chestnut St when DANG stopped, HUTCHINSON attempted to stop and his 4wd engaged and he slid slightly into the back of DANGS car, no marks or damage to either car. DANG elected to go to NWH with Cataldo . DANGs veh towed so as not to obstruct roadway.

MV#1 Stated while traveling westbound down Commonwealth ave he struck a patch of ice. The ice caused his vehicle to rotate 360 degrees and proceeded to slide on the roadside shoulder striking a verizon poll ( 102/285). MV#1 traveled approximately 12ft from the road before striking the poll. MV#1 sustained heavy front end damage. MV#1 had all airbags deployed and had a small fire under the front end. I observed multiple other vehicles passing having difficulties and vehicles sliding due to significant ice conditions. Driver of MV#1 had injuries to his left arm, possible broken forearm/wrist. Passenger of MV#1 stated she had chest pains. Medics transported both parties to Newton Wellesley Hospital for further evaluation. Tody's Towed the vehicle due to heavy front end damage.

Narrative:
At approximately 0110HRs on Sunday February 17, 2019, I was dispatched to the area of Dedham St. and Spiers Rd. (both public ways in the City of Newton), for a report of a motor vehicle vs pole accident with injury, airbag deployment and entrapment. Upon arrival, I exited my cruiser with my medical bag and AED and went to assist the motorist involved in the crash. MV1 (MA Passenger: 255ZA9) was completely off the roadway and on the shoulder of the road (facing Eastbound) with its nose contacting the light pole ( Verizon Pole #:103/80), which was now broken and listing to the side. Attempts to open the doors were unsuccessful and I attempted to communicate with the operator through a partially broken driver's window frame which was bent. The operator was slumped forward with his hand cradling his head and he was unable to speak, but was breathing. Newton Fire arrived and attempted to gain access to the vehicle via several manual tools with no success and instead deployed the Jaws of Life which was successful. Newton Fire also made entry into the rear passenger compartment because we viewed a child's car seat and checked with negative results for any child in the vehicle. Once the operator was cut out of his seatbelt and safely out of MV1 and on board Newton Medic 1, I entered the ambulance where he was identified as Mark Topham. It should be noted that Topham was having difficulty answering basic questions such as his name, date of birth, today's date, Etc. At one point when he was asked what today's date was, he gave his birth date of February 21, 1983 and it took him several attempts to say his last name, I had to run him over the air with a partial name to ascertain who he is. Also of note, in my presence and that of Sgt. Devine, Topham informed the medics that he had been consuming alcoholic beverages prior to the crash. Upon viewing the scene, I noted that the roadway was lit and I did not observe any skid marks on the dry, ice-free payment, indicating that there was unlikely any braking or swerving to avoid the collision. I spoke to the reporting party, Lindsay Giardina, she stated that she was traveling on Dedham St. when she heard what she believed to be a motor vehicle crash. When she came upon said crash, she attempted to assist the operator, as she was an off duty firefighter. She was unable to gain access to the inside of the vehicle, but communicated to the operator in the same manor as I did. She stated that she was able to smell the odor of an alcoholic beverage emanating from the operator. Topham was transported to Beth Israel of Boston, where he was assigned patient record number 3109921. I attempted to speak with him at the hospital, but was unable to do so because of his current state; I noted several injuries on Topham, such as a bloody lip, a wound to his lower left side and a wound to his left leg. Due to Topham's statements onboard of Medic 1 and Lindsay Giardina's observations, I will be charging Topham with one count of violating MGL Ch.90 S. 24 K OUI Liquor 2nd Offence, one count of violating MGL Ch.90 S. 24 E Negligent Operation of a Motor Vehicle and one count of violating MGL Ch. 89 S. 4A Marked Lanes Violation via MA Criminal Citations which I placed in the mail. Additionally, I will be seeking a subpoena for Topham's blood work from the hospital. Note: The crash occurred on the Newton/Boston line, Newton dispatch's GPS indicated that the crash was in Newton. I took several pictures of the scene and have forwarded them to IT to be attached to this report. I completed and filled an NPD Motor Vehicle Tow Inventory Form and Officer Boudrot stood by for the tow.

On February 27, 2019 at approximately 06:33 hours I responded to the area of 1091 Centre St for a motor vehicle crash with minor injuries. Upon arrival, fire and medics were evaluating all parties involved. The operator of MV2 complained of neck and back pain but refused medical transport but will be seeking care if pain continues. All motor vehicles were traveling Northbound on Centre St. slowing in traffic. MV1 crashed into the rear MV2, which in turn made MV2 crash into rear of MV3. MV1 admitted fault, claiming that she hit the gas pedal instead of the brake. MV2 was towed by the scene by Tody's.

Narrative:
On Friday March 15th 2019 at approximately 0730 hours I responded to the intersection of Beacon Street and Herrick Road for a report of a hit and run involving a bicyclist. The weather at the time of the accident was cloudy and the road conditions were dry. Both streets are public ways in the City of Newton. Upon arrival I observed the victim identified as Glenn Butcher (MA OLN S36552335), inside the ambulance getting medical attention. Butcher was getting a bandage applied to his left leg and complained of leg, back and neck pain. Butcher appeared visibly shaken and in pain. Butcher stated he had the green traffic light and was travelling eastbound on Beacon Street crossing Centre Street and then continuing straight crossing Herrick Road. Butcher stated MV1 was travelling westbound on Beacon Street and took a left turn onto Herrick Road and collided into his bicycle. Butcher states upon impact he flipped over his bicycle and landed on the hood of MV1, before rolling off and landing on the ground. Butcher states he was wearing his helmet which was present. Butcher states his bicycle ended up partially under MV1. Butcher states the operator of MV1 pulled his vehicle to the side of the road and got out of his vehicle and spoke to several witnesses briefly. Butcher states the operator then left the scene without properly exchanging information and did not wait for the police to arrive. Butcher described the vehicle as a dark gray Honda, and described the operator as an older white male, 5'10 approximately 80 years old with white hair. Butcher was also able to provide an email address for another bicyclist who witnessed the accident. The witness left his information with Butcher prior to my arrival. Fire fighters on scene were able to get the registration number of the vehicle from the witness (MA REG 4CX148) and gave it to Officers on scene. Butcher was transported by Medic 1 to NWH with minor injuries. His bicycle had visible damage to the frame area and skid marks were present on both tires. 19 photographs of the scene and the bicycle were taken and sent to the IT dept. email. Officer Gabriel transported the bicycle to the station where it was logged into Property & Evidence for safekeeping. Dispatch was able to query the registration which came back to a gray, 2010 Honda Accord registered to a Matthew Sugarman (MA OLN S38601397), out of W. Roxbury. Dispatch contacted the Boston Police Dept. and had them send an Officer to the address to try to make contact with Sugarman. Sugarman was advised to contact the Newton Police Dept. and I was able to speak with him on the telephone. Sugarman states he was driving the vehicle today and was involved in an accident with a bicycle. Sugarman states he was travelling westbound on Beacon Street and was taking a left onto Centre Street not Herrick Road. While speaking with Sugarman he seemed confused as to his location during the accident and was adamant that he was turning left onto Centre Street and not Herrick Road. I advised Sugarman that I was on scene with the first responding Officers and the accident clearly occurred at the intersection of Beacon Street and Herrick Road based upon the final resting place of the bicycle. Sugarman went on to state that his vehicle was already turning when the bicycle struck his vehicle and he believed it was the bicyclists fault. I advised Sugarman it appears the bicyclist had the right of way since it was traveling straight and that because his vehicle was taking a left turn, he failed to yield for on coming traffic at an intersection. When asked why he did not stay at the scene of the accident Sugarman stated he got out of his vehicle and spoke to the bicyclist and he "seemed okay". Sugarman states he then left the scene while the victim was still sitting on the ground. I asked Sugarman if he had ever been involved in accident before and he replied "no." I advised Sugarman that I am currently looking at his drivers history and see nine accidents that he was involved in. Sugarman stated "he has never been in accident like this before." I then asked Sugarman what the law states when you are involved in an accident and he was not able to provide me with an answer. I advised him that the law states you must stop and exchange information before leaving the scene of an accident. Again he stated the bicyclist seemed okay and "he had somewhere to be so he left." Sugarman has an extensive drivers history including nine surchargeable accidents, reckless operation, two different charges of leaving the scene property damage and two immediate threat affidavits filed against him. I advised Sugarman that I will be mailing him a citation for Ch89 s8 Fail to yield to oncoming traffic, and Ch90 s24 Leaving the scene of personal injury. I also filed an immediate threat form with the registry. I emailed the witness using the information he provided to the Newton Fire Dept that was on scene and received a phone call back from the witness identified as Bruce Adams. See witness report for his statements. I contacted the Bank of America located at 808 Beacon Street in Newton, as they are the closest business to that intersection with video surveillance. I spoke with George Walsh (781-834-8275) who stated they do not have any working cameras on that side of the building to the bank. For this report the bicyclist is entered as a non-motorist for validation purposes. Witness Bruce Adams (MA OLN S96398108) stated that he was riding his bicycle travelling behind the victim about 100 feet away. Adams states he was watching the victim and had a clear view of him as he travelled behind him, through the green traffic light heading eastbound on Beacon Street. Adams stated he witnessed MV1 travelling westbound on Beacon Street take a sharp left turn onto Herrick Road which turns into Union Street. Adams stated that the vehicle turned left colliding into the bicycle and there was no possible way the bicyclist had time to stop to avoid the collision. Adams stated the victim flew over his handle bars and fell on top of MV1's hood and then landed on the ground. Adams stated the victim's bicycle was stuck under MV1's front bumper. Adams stated the operator of MV1 backed his vehicle up with the bicycle still attached to his vehicle bumper and pulled MV1 over to the side of the road. The operator then got out of his vehicle and walked up to the victim who was laying on the ground visibly in pain. Adams stated the operator asked the victim if he was okay to which the victim did not immediatley reply as he was visibly shaken up. Adams stated the operator then walked back to his vehicle and attempted to leave the scene and that is when Adams stood infront of MV1 to stop him from leaving the scene. Adams stated he then removed the bicycle from underneath the bumper of MV1 and placed it on the side of the road. Adams stated the operator got out of his vehicle again and walked up to the victim and asked him "what do you want to do?". Adams stated the victim did not respond and that is when the operator got back into his vehicle and drove away, leaving the scene before any emergency response arrived. Adams stated he was able to memorize MV1's registration and stayed with the victim until police, fire and medics arrived. He stated he gave the fire dept the registration and his contact information prior to leaving the scene.

On the above date/time I responded to the area Dedham St/Parker St for a report of two motor vehicle crash with minor injuries. Upon arrival with Fire/medics I spoke to the oper of v1 Ms. Hollowell who had a shoulder injury due to side impact. She stated she was traveling on Dedham Street (east) applied her left turn signal and was turning left onto Parker Street when v2 went through the stop sign and struck her vehicle. I spoke to the operator of v2 Ms. Kuznetsova who stated she was stopped at the stop sign on Parker St ( south) She stated she accelerated forward turning left (east) onto Dedham St when vehicle one struck into her vehicle. Oper of v1 signed a patient refusal and oper of v2 had no injury. Both parties were unsure of the area in the intersection of impact, all vehicles were moved by the operators prior to my arrival and I could not find any physical characteristics in/ on the roadway to find impact area. Therefore the diagram reflects the damage points on the vehicles but not the impact area on the roadway. All parties advised of the process.

On the above date/time I responded to the area of Cherry Street and Webster Street for a three motor vehicle crash with injuries. Upon arrival along with Sgt. Dempsey, Fire/medics. I spoke with the operator of vehicle one. He stated he was stopped on Webster Street facing westbound a the red traffic signal. He further stated the traffic signal on Webster Street turned green, he accelerated forward. He stated vehicle two traveled north on Cherry Street at a high rate of speed through the red traffic signal. He stated he had no reaction time due to the speed of vehicle two and had impact. I spoke to the operator of vehicle two. He stated he was traveling north on Cherry Street going too fast, he stated he thought it was a school zone so he slowed down and he did not observe the color of the traffic signal. He stated he observed vehicle one come out of the Webster Street at the last second at impact. I spoke with the operator of vehicle three. She stated only remembered being stopped on Cherry Street facing south. She stated she did not remember what color the traffic signal was, just knew she was stopped when she felt heavy impact from vehicle two. Injuries: Oper of V1 none at this time- patient refusal Oper of V2 none at this time - patient refusal Oper of V3 Head laceration and head trauma, transported to Newton- Wellesley Hospital Tody's towing arrived on scene and towed vehicles two, three to their tow lot. All proper paper work was done for a towed vehicle(s). All parties advised of the process. After the vehicles were towed, and the scene was cleared I watched the cycling of the traffic signals in the intersection and observed they were functioning properly. It appears a this time based on statements and physical characteristics of the impact area on the roadway, that vehicle two went through the red light on Cherry Street, which caused the crash.

Operator of vehicle one ( MA reg 3986BV 2012 MERZ ) , Vladimir Krapchev stated that on 03/21/2018 at 08:56 hours he was stopped at a red light on Center St at Commonwealth Ave. Krapchev stated that his vehicle was heading south on Center St and was the third vehicle in line to turn west bound on Commonwealth Ave. Center St and Commonwealth Ave are both public ways in the city of Newton. Krapchev stated that " a red/maroon full size sedan fire department car " struck the front driver's side fender of his vehicle. Krapchev stated that the fire department car was travailing south bound on Center St in the north bound lane, passing stopped south bound Center St traffic. Krapchev stated that there was also north bound traffic on Center St that was turning from the west bound lane of Commonwealth Ave. Krapchev stated that the fire department car turned onto the east bound lane of Commonwealth Ave and did not stop. Operator of vehicle two Newton Fire Department LT Phillip McCully stated that he was driving Newton Fire Department Car 14 ( MA reg MVN MF461 2004 Ford Crown Victoria ) south bound on Center St with his vehicle's emergency lights and siren activated. Lt McCully stated that he was responding to a motor vehicle crash with possible entrapment on Hammond Pond Parkway. Lt McCully stated that he was passing stopped Center St south bound traffic in the north bound lane of Center St. Lt McCully also stated that there was north bound traffic on Center St and " it was tight between the cars ". Lt McCully stated that he was unaware that a crash had occurred. Lt. McCully stated that he did not fell and contact between his vehicle and Krapchev's vehicle. Lt McCully also stated that he did not hear the crash because of his vehicle's siren was activated. Krapchev's vehicle had minor damage and red paint marks on it's front driver's side fender. The Fire Department car had scuff marks on it's passenger side rear door. Neither vehicle was towed and both operators stated that they were not injured in the crash. I took photos of both vehicles and forwarded the disk to the NPD It bureau for down loading.

On 4/14/2019 at approx 0923Hrs while assigned to 497 I responded to the area of Commonwealth Av and Ash St for a report of a three car crash. upon arrival I observed three cars off to the side of the road. i spoke with the operator of ma Reg T6T, Robert MURPHY who stated he had stopped for the red light at Commonwealth and Ash St when the operator of TN Reg 2N37P1, Jennifer BURNELL failed to stop striking the operator of Ma Reg 8CL834 Evan GUIDRY who in turn than ran into the back of MURPHY. I spoke with BURNELL who stated she was lost and had been looking down onto her phone looking for a map when she ran into the back of GUIDRY. BURNELL cited for violation MGL90/13 distracted Operation , GUIDRY transported to NWH by Cataldo Ambulance for neck and head pain. GUIDRY and BURN ELLS vehicles towed by Todys. On Thursday 04/18/209 at approximately 1222 hours I responded to 81 Oak Street for a motor vehicle accident involving Newton Fire Ladder 2. At the time of the accident the weather was cloudy and road conditions were dry. Oak Street is a public way in the city of Newton. Upon arrival I spoke with the operator of MV1 who stated while trying to manuever through oncoming traffic on Oak Street heading southboud MV1 struck MV2's drivers side mirror while MV2 was parked. MV1 reported no damage and reported no injuries. MV2 reiterated the same events and reported very minor damage to the drivers side mirror. No injuries were reported. Several pictures were taken of both vehicles and submitted to the IT Bureau.

On Thursday 04/18/2019 at approximately 1400 hours I responded to 37 Elm Street for a single vehicle accident with City of Newton property damage. At the time of the accident the weather was cloudy and the road conditions were dry. Elm Street is a public way in the city of Newton. The operator of MV1 identified as Silton Barrett (S17437114) stated he parked his UHAUL van in front of 37 Elm Street to make a delivery. While attempting to pull MV1 back into the traffic lane he struck a fire hydrant knocking it over and completely detaching it from the base on the ground. MV1 sustained minor front end damage and was able to leave the scene. Several photographs were taken and submitted to the IT Bureau. City of Newton was notified for the property damage.

I responded to the area 107 Dartmouth St. at approx. 0345 on 04/28/9019 along with N493 Officer Kelly Donahue and N508 Sgt. Devine for a report of an MVA. The MVA was reported to be single vehicle crash. The vehicle left the roadway and was on the front lawn of 107 Dartmouth St. I arrived on location (first on scene ). I observed tire tread marks across the front lawn of 107 Dartmouth St. I then observed a vehicle in the bushes toward the left front side of the house. The vehicle; NY Reg. JHE8252, Porsche color blue. The RMV info on the plate came back registered to Porsche;of ;South. Registration plate type: Dealer Assignment. 185 W Sunrise HWY; Freeport,NY (no further info). The vehicle was unoccupied and secured. I felt the hood of the car which was still warm from the engine. I checked the immediate area surrounding the crash scene for an operator but found no one. I radioed to other responding units to canvas the neighborhood and surrounding area for the operator on foot. I spoke to the homeowners at 107 Dartmouth St., identified as Anthony and June Limberis. Mrs. Limberis stated she heard a load noise and a car engine revving. She stated when she looked out the window she observed a W/M tall, thin, wearing tan khakis and a light colored plaid shirt walking across the front lawn. He proceeded up Dartmouth St. (northbound) toward Commonwealth Ave. Newton Fire Dept arrived on location to check the crash scene and vehicle for possible leaking fluids and any other hazards. As I was talking with the firefighters we were approached by two men. One of the men was intoxicated. He was unsteady on his feet, had glassy bloodshot eyes, and was slurring his speech. The man was a W/M tall/thin approx. 6'2" and was wearing tan khakis and a light colored plaid shirt. He was identified as Eric Adams DOB 09/18/1969. The second man with him was identified as John Adams DOB 02/07/1936; An elderly W/M approx. 5'6" also wearing tan khakis and a light colored plaid shirt. Eric Adams stated his father John was the operator of the vehicle. Eric Adams had a strong odor of Alcoholic beverage emanating from his person and breath. He continued to slur his speech and was unsteady on his feet as he spoke. He was swaying back and forth and side to side as he spoke to me. Officer K. Donahue and Sgt. Devine were on scene at this time and also made the same observations. Multiple times we had to grab hold of Eric Adams to keep him from losing balance and falling. We spoke with Eric and John Adams for several minutes as their accounts of what took place were not consistent. I asked Eric Adams where the keys to the vehicle were. Eric Adams immediately pulled the starter key to the vehicle from his front right pocket. Eric Adams became defensive and belligerent when questioned about who was operating the vehicle. John Adams appeared tired (as if he just woke up) and confused as to the situation. Sgt Devine and I both spoke with Eric Adams and John Adams. Although both men claim John Adams was the operator of the vehicle, a clear determination was uncertain. A query of both mens driver's license status on NY Licenses came back active. Eric Adams has two prior DUI's on his RMV record. Officer Donahue and Sgt. Devine escorted the two men to 93 Dartmouth St. Eric Adams stated they were staying at that residence which is his sister's house. I stayed at the crash scene to document information and assist with clearing the MV from the property with Tody's Towing Service. As they were walking away I observed Officer Donahue and Sgt. Devine continue to have to assist Eric Adams with staying on his feet. Approximately ten minutes later as I was speaking with the aforementioned homeowners Mr.&Mrs. Limberis. Officer Donahue and Sgt. Devine returned to the crash scene. As they were updating me we observed Eric Adams returning to the crash scene. He was stumbling and swaying as he walked and repeatedly almost fell. Officer Donahue and Sgt. Devine immediately intercepted and assisted him before he fell. As they were assisting him again I continued speaking with the homeowners. Suddenly I heard Officer Donahue and Sgt. Devine struggling with Eric Adams who was now on the ground and resisting being handcuffed. I ran to assist them. Eric Adams continued to resist and be uncooperative until he was appropriately restrained in handcuffs. Eric Adams was placed under arrest by officer Donahue for Disorderly Conduct and Resisting Arrest. Reference Officer Donahues Newton Police Incident Report #19016484. The vehicle NY Reg. JHE8252 was removed from the property of 107 Dartmouth St. and towed from the crash scene by Tody's Towing Service.

On Sunday, May 5th, 2019 at approximately 1315 hours while working the City of Newton detail for the Walk for Hunger I was standing at my assigned post at the intersection of Centre Street and Willow Street. At the time, the road surface was dry and the weather was cloudy. Traffic was particulary heavy as there were several streets in the area still blocked off for the Walk for Hunger and there were participants still walking the route in the area. My attention was drawn to the intersection of Centre Street and Bowen Street as there was a motor vehicle travelling southbound on Centre Street with their left turn signal on attempting to take a left turn on to Gibbs Street. Gibbs Street was temporarily blocked to non-residential motorists as there were several pedestrians in the road crossing over Gibbs Street. As the motor vehicle attempted to turn, there were several pedestrians in the crosswalk crossing over Gibbs Street. The vehicle had stopped halfway into the northbound lane on Centre Street blocking on coming traffic. The operator appeared to realize Gibbs Street was blocked and attemped to turn the vehicle back into the southbound lane to continue southbound on Centre Street. As the vehicle turned back into the southbound traffic lane, the vehicles rear end collided with Officer Richard Clements, who was operating NPD motorcycle unit 533. Officer Clements was traveling southbound on Centre Street and was passing the vehicles in the southbound traffic lane which were currently all at a standstill. Officer Clements had his emergency lights activated as he was on his way to remove traffic cones from the street to open up all the traffic lanes along the event route. Officer Clements was working the City of Newton detail for the Walk for Hunger assigned to the motorcycle to assist with traffic. Officer Clements front wheel, left side of the motorcycle and left leg collided with the rear end of the motor vehicle. The impact of the crash caused him to fall off the left side of the motorcycle and land on the street face first. At this time I immediately ran from my post to Officer Clements location and radioed dispatch that a motorcycle officer was down and that I needed assistance. As I approached Officer Clements he was face down, unconcious and breathing. I was able to roll Officer Clements into the recovery position and at this time Officer Clements regained conciousness and was able to speak and stated " What happened? What was that lady doing she turned right into me". I advised Officer Clements that he had just been involved in an accident and to stay still and that help was on the way. The operator of the motor vehicle identified as Helen Leibner (S52717988) stopped her vehicle immediatley after the crash and got out of her vehicle to offer her assistance. I advised Helen and several other witnesses and bystanders who were now crowding around Officer Clements and I to back away from the scene and to not move any of the vehicles involved. Officer Selig arrived on scene and was able to help crowd control until further help arrived. Helen and all witnesses were advised to stand by for police to take their statements. While tending to Officer Clements he was disoriented and made several attempts to move his body to which I had to physically hold him still to prevent possible further injuries. While speaking with Officer Clements I noticed blood coming from the inside of his mouth. Officer Clements was in a noticeable amount of pain. He complained of head, back and left leg pain. Officer Clements was wearing a yellow ANSI certified Newton Police reflective jacket and a Newton Police issued motorcycle helmet. Once the paramedics and fire personnel arrived I was able to speak to the operator of the motor vehicle and several other witnesses. Helen stated she was traveling southbound on Centre Street operating her blue 2014 Audi A4 (MA REG 213BC9). Helen states she was attempting to take a left turn onto Gibbs Street and activated her left turn signal. Helen states she had stopped halfway into the northbound lane as several pedestrians were crossing Gibbs Street in the crosswalk at the time. Helen states after the pedestrians cleared she saw the traffic cones blocking the street and realized the street was closed. Helen states she turned her vehicle back into the southbound traffic lane and did not see Officer Clements or the motorcycle until impact. Helen was not injured and her motor vehicle sustained moderate rear end damage. Her motor vehicle was able to drive away from the scene. Witness Adron Gordon (S90357161) stated he was participating in the Walk for Hunger and had just crossed over Gibbs Street when the crash occured. He stated he witnessed the Officer on the motorcycle with the emergency lights on traveling southbound on Centre Street behind the motor vehicle. The motor vehicle stopped in traffic attempting to turn left onto Gibbs Street. He states then the vehicle made a slight turn back into the southbound traffic lane and the motorcycle collided with the rear end of the motor vehicle. He states the Officer then fell off the motorcycle landing in the middle of Centre Street. Witness Elizabeth McCarthy (S00242346) and Katherine Regan (S43486601) stated they both were in the same vehicle together stopped in traffic in the northbound lane of Centre Street just before the set of lights at Centre Street and Bowen Street. Both witnesses stated they do not remember if the driver of the motor vehicle had the turn signal on but they did see her attempt to turn onto Gibbs Street. They stated they also saw the motorcycle with emergency lights activated travel southbound on Centre Street approaching behind the vehicle. Witnesses state the motor vehicle then suddenly turned back into the southbound lane and the motorcycle collided with the passenger side tailight of the motor vehicle. Both witnesses state the motorcycle attemped to turn away from the vehicle to avoid the crash but was unable to do so. Witness Nina Wang (S39460985) stated she was parked on the southbound lane of Centre Street just prior to the crash scene. Nina states she was in her City of Newton DPW marked vehicle with the hazards on as she was working to assist with traffic safety for the Walk for Hunger. Nina states she witnessed the motorcycle travel southbound on Centre Street and pass the vehicles stopped in the traffic lane. Nina states the motorcycle passed the traffic using the left side and had the emergency lights activated. Nina states the motorcycle was just passing through the green light at Centre Street and Bowen Street when the motor vehicle that was stopped at the lights attempting to take a left turn onto Gibbs Street. Nina states the vehicle then abruptly jerked her car to the right and collided with the motorcycles left side. Nina states she heard a loud crash and witnessed the Officer fall off the motorcycle and land face first onto the street. The motorcycle's final resting place was on its left side approximately two feet away from where Officer Clements was located. The motorcycle sustained moderate damage to its left side, including the front wheel well and undercarriage. The motorcycle left scuff marks on the street which were measured and photographed. Several photographs of the scene and damage to both vehicles were taken and submitted to the IT Bureau. The motorcycle was towed by Todys. Centre Street and Bowen Street are both public ways maintained by the City of Newton. The injured motorcyclist, Officer Clements, was transported to Saint Elizabeth Hospital with fire fighters on board. I was able to follow up with Officer Clements on the phone the next day who was admitted over night at the hospital. Officer Clements stated he has no recollection of the accident and can only remember driving down Centre Street to begin to pick up cones to open up all lanes of traffic. Officer Clements stated he suffered a brain bleed, head, back, left foot and right thumb injuries. A Massachusetts Uniform Citation was issued and mailed to Helen for Chapter 89 Section 7A, failure to yield to emergency vehicle, police motorcycle.

Operator of MV1 stated he was traveling southbound on Centre Street and came to a stop to allow three pedestrians and a dog to cross at the crosswalk at Allerton Rd. Operator of MV2 stated he observed MV1 stopping to allow people to cross and he came to a complete stop as well. Operator of MV3 stated she did not see MV2 stopped and did not see his brake lights on. MV1 said that when the pedestrians were about three quarters of the way crossed, he heard a loud crashed and saw MV2 swerve into the on coming traffic lane. MV2 made slight contact with the rear of MV1 causing scratches to the rear left bumper. MV2 said MV3 struck him from behind causing him to push forward. He swerved into the on coming lane to avoid hitting MV1. He continued to turn to the left in order to avoid the pedestrians that were still crossing. No contact was made with them. Newton Fire Engine 3 and Cataldo PB3 responded. MV2 was towed by David Donahue from Todys Towing. It sustained major rear end damage. The operator of MV2 was evaluated by Cataldo PB3 and signed a patient refusal. MV3 was towed by Bobby Donahue from Todys Towing. It sustained heavy front end damage, front air bag deployment and a small spill from the engine block. Operator of MV3 was evaluated and transported to NWH by PB3 for minor injuries. The three pedestrians witnessed the accident. All three stated that MV1 stopped to allow them to cross and they saw MV2 also come to a complete stop. MV3 struck MV2 from the rear causing him to swerve into the on coming lane to avoid hitting MV1 and them.

On 06/08/2019 at 0150 hours I received a call from Sgt Lee in regard to a head on motor vehicle crash with entrapment that had occurred on Center St at the four way intersection with Cabot St and Sergeant St. Upon my arrival at the crash scene I was advised by Sgt Lee that all occupants of the involved vehicles had been transported to hospitals for treatment. I made the following observations of the crash scene , Center St is a two lane public way in the city of Newton that runs north to south. I observed a city of Newton contracted Cataldo ambulance stopped in the south bound lane of Center St at Cabot St. The ambulance was MA AMN registration AMB 370 a 2017 Ford Econo Line that had heavy damage to the front end and driver's side fender. I observed that the front tire of the ambulance was snapped off and was pushed back towards the driver's side door. I further observed that both of the front and side air bags of the ambulance were deployed. The front of the ambulance's final point of rest was 8'8" from a fixed overhead light pole on the northeast corner of Center St at Cabot St. The ambulance's front passenger side tire was 4'3" front the curb on the Cabot St side. The ambulance's rear passenger side tire was 2'3" from the Cabot St side curb. The ambulance's rear driver side tire was 5'1" from the double yellow line on Center St. The ambulance's front driver side tire was 6'3"from the Center St double yellow line. The width of Center St from the double yellow line to the Center St curb is 15'' and 16'" to the Sergeant St side. I observed a 2011 Mazda 3 with MA PAN registration 6KJ783 facing south bound in the north bound lane. The Mazda had heavy damage to it's front end, and I observed that it's front tires had snapped off. The Mazda's roof had been removed by Newton Fire to extract the occupant. The front of the Mazda's final point of rest was 40'3" from a fixed overhead light pole on the northeast corner of Center St and Sergeant St. The Mazda's front passenger tire was 4'6" from the double yellow line on Center St. The Mazda's rear passenger side tire was 7'6" from the Center St double yellow line. The Mazda's rear passenger side tire was 1'2" from the Center St double yellow line. The Mazda's front driver's side tire was on the curb on the Sergeant St side of Center St. There were no skid marks from the Mazda which indicates that no braking occurred. At the time of the crash, the Cataldo Ambulance was being operated by Paramedic Corey Bohan and Paramedic Tom White was in the front passenger seat. Bohan stated that he was operating his vehicle southbound on Center St towards Cabot St after clearing a medical call. There were no patients in the ambulance at the time of the crash, and Bohan estimated he was travelling 25 MPH . Bohan stated within less than two seconds he observed a vehicle traveling towards him northbound on Center St in the southbound lane and estimated it's speed to be approximately 80 to 100 MPH.Bohan stated he was able to slow to a near stop and had moved his vehicle towards the right shoulder on Center St before the Mazda struck his vehicle head on. Bohan stated that after forcing his way out of the driver's seat of his vehicle, he rendered medical aid to the operator and only occupant of the Mazda, Jessica Pondish. Bohan stated that Pondish speech was slurred and he detected an odor of an alcoholic beverage emanating from her breath and person. Bohan was later transported to Newton Wellesley Hospital for evaluation. The passenger in the ambulance, Tom White stated to officers that he was working on the ambulance's laptop computer and did not see anything leading up to the impact from the crash. White stated that he was able to force himself free of the passenger side of the ambulance, and rendered medical aid to Pondish. White stated that Pondish was " in and out of it " and did not appear coherent.White stated that he asked Pondish if she had consumed any alcoholic beverage and she nodded in the affirmative. White Stated that he was unsure if Pondish understood his questions. White stated that he did not detect any odor of an alcohol beverage emanating from Pondish's breath or person. White was transported to Newton Wellesley Hospital for evaluation. Pondish was freed from her vehicle after approximately 22 minutes by Newton Fire Department units by utilizing special extraction tools. Pondish was transported by Newton Medic Unit Two to the Brigham and Women's Hospital in Boston for treatment. From 06/08/2019 to 06/10/2019 Pondish had two surgeries and was unable to speak due to her condition. On 06/10/2019 myself and Officer Gaudet were able to speak with Pondish in her hospital ICU room at the Brigham and Women's Hospital. Pondish's parents Lorena and John Pondish were present while we spoke with her. Officer Gaudet asked Pondish about the events that led up to the crash. Pondish stated that on 06/07/2019 she was at a friend's residence in Norton, MA. Pondish stated that while at her friend's residence she consumed a bottle and a half of Riesling wine. After Pondish's admission myself and Officer Gaudet stopped our conversation with her. I read Pondish her rights directly from a Miranda card and explained them in full before allowing her to continue speaking with myself and Officer Gaudet about the events of that led to the crash. Pondish stated that she understood her rights. Officer Gaudet asked Pondish if she had consumed any alcoholic beverages prior to driving home to her residence in Newton. Pondish stated that she drank a bottle and a half of Riesling wine by herself between the hours of 1700 and 0000 hours on 06/08/2019. Officer Gaudet asked Pondish how large the bottles of wine was that she drank, and she stated that they were average size. Officer Gaudet asked Pondish how she perceived her state of being was after consuming a bottle and a half of wine. Pondish stated that she felt " buzzed " . Pondish stated that she left her friend's residence in Norton at approximately 0100 hours on 06/08/2019 to return to her residence in Newton. Pondish stated that she does recall the events leading up to the crash, except that she tried to make a phone call but the person did not pick up. Pondish stated that her seat belt was on and that she was not texting while operating her vehicle. Pondish stated that her injuries consisted of a broken left elbow and internal injures to her intestines. On 06/09/2019 I spoke to Cataldo Paramedic Robert Canning and Stephen Heirholcer who were the medic unit that transported Pondish to the Brigham and Women's Hospital. Canning stated that he rendered medical aid to Pondish for approximately 20 minutes while Fire units worked to free her. Canning stated that Pondish's legs were pinned under her vehicle's dashboard. Canning stated that during the approximately 20 minutes that he was in close proximity to Pondish he could detect a strong odor of an alcoholic beverage that was emanating from her breath and person. Canning stated that once Pondish was freed he continued to treat and monitor her during the transport to the Brigham and Women's Hospital. Canning stated that when he asked Pondish if she had consumed any alcoholic beverage she stated that she had. Paramedic Heirholcer stated that he did not detect any odor of an alcoholic beverage emanating from Pondish's breath or person. Based upon the physical evidence at the crash scene, and statements made to officers by all of the above mentioned persons, Pondish is being charged with violation of MGLs 90/24j OUI liquor, 90/24e negligent operation of a motor vehicle, 89/4a failure to stay within marks lanes, and 90/17 speeding. On 06/12/2019 at the conclusion of the crash investigation I issued MA Criminal Application T 1269996 in hand to Pondish at her hospital room and explained to her what action she needed to take with court. On 06/11/2019 a letter of preservation was sent to Brigham and Women's Hospital requesting that a sample of Pondish's blood, urine and medical records be saved. On 06/12/2019 Brigham and Women's Hospital confirmed that a sample of Pondish's blood had been preserved. As of 06/12/2019 a search warrant for Pondish's blood sample was pending. Photos of the involved vehicles and of the crash scene were taken and downloaded by the NPD's IT Bureau. Both involved vehicles were towed from the crash scene and towed motor vehicle forms were filled out. An Iphone and backpack belonging to Pondish were taken from her vehicle for safekeeping and returned to her on 06/11/2019. A copy of this report was faxed to the MA RMV Driver Control Unit to request an immediate threat suspension.

At approximately 0010HRs on Thursday, June 13, 2019, I was dispatched to 141 Dedham St. for a report of a two car crash. Upon arrival, I observed MV1 (NH Pass:3266403) to be parked on the side of the roadway in front of the aforementioned address and it appeared to have been rear ended by MV2 (MA Pass: 513WA3). Being that MV2's airbags were deployed, I requested Newton Fire to evaluate the operator of MV2, who was ultimately medically cleared and signed a patient refusal. Upon speaking to the operator of MV2, he stated that he was traveling Northbound on Dedham St. and feared that he was coming too close to the solid yellow divider lines and over steered to the right, accidently hitting MV1, which was parked. The owner of MV1 was on scene and stated that he was inside and came out to find the crash. I noted damage to the driver's side quarter panel of MV1 and damage to the front end/hood area of MV2. Todys serviced arrived and took custody of MV2. I completed/filled an NPD Towed Motor Vehicle Form. I cleared without incident.

Operator # 1 stated that he was coming to a stop in traffic while driving SB on Centre Street at Hyde Street when he was rear ended by vehicle #2. Operator #2 stated that he was driving SB on Centre Street, he saw the vehicle in front of him stopping. He attempted to stop but wasn't able too stop in time. He stated his car wouldn't stop due to the wet road. NFD was able to separate the two vehicle which were stuck by the tow hitch. Medics got 3 refusals. Vehicle #2 was towed.

On 06/26/2019, while assigned to N494, I, Officer Conary, responded to Cabot Street at Blake Street for a motor vehicle accident involving a City of Newton vehicle, fire truck. Operator of MV1 stated that he was traveling Northbound on Blake Street taking a left turn onto Cabot Street. Cabot Street is under construction and there was barrels in the road which caused MV1 to go around them. MV1 was to close to MV2 while taking the turn and hit MV2. Operator of MV2 stated that she was stopped Eastbound on Cabot street, waiting to take a right turn onto Blake Street. When MV1 was turning, it was too close and struck her vehicle. All parties were offered and declined medical attention. MV2 had to be towed from scene. Pictures were taken and will be submitted to IT accordingly.

On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

Operator of MV3 stated he stopped at the stop sign on Lake Ave. then attempted to continue Northbound through the intersection. Operator of MV3 stated as he attempted to proceed through the intersection MV1 was flying down Beacon St. and collided with his vehicle. No injuries. Newton Fire and medics responded on scene and all parties involved signed patient refusals. Todys responded and towed MV1 and MV2. MV1 sustained substantial drivers side and left front end damage. MV2 sustained substantial left front end damage. MV3 sustained minor front end damage. It should be noted as I arrived on scene of the accident MV3 was in the middle of the intersection facing Northbound as indicated in the crash diagram. A towed motor vehicle form was filled out for both vehicles in regards and placed on file All debris from the accident was clear from the road including the bike lane.

On Thursday, August 8, 2019, while assigned to Traffic unit N525, I responded to the area of Centre Street and Mt. Ida Street for a report of a 3 car motor vehicle crash with entrapment. The weather at the time of the crash was clear and sunny. The road surface was dry. Centre Street and Mt. Ida Street are both public ways maintained by the City of Newton. On arrival, I was updated by Sgt. Chisholm that one of the vehicles (MV3) had a female operator that was entrapped as a result of the collision. She was freed from the vehicle by the Newton Fire Department and transported to Beth Israel Hospital. Another operator (MV2) was transported to Newton Wellesley Hospital with minor injuries. A third operator (MV1) remained on scene with no injuries. With the assistance of Officer Wade we conducted a survey of the crash scene. We observed a large debris field at the intersection of Centre Street and Mt. Ida Street with debris from all three vehicles. The point of impact was observed to be on Centre Street (N) before Mt. Ida Street. There were fresh scrapes in the pavement as a result of the colission. We also observed skid marks from MV1 measuring 57 feet 8 inches from the start of it's skid to the vehicles final rest MV1 crashed into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane of Centre Street and crash into MV3 as it was passing by. The impact from MV2's front end crashing into MV3's driver side resulted in the operator of MV3 becoming entrapped. MV2 and MV3 came to final rest on Wesley Street (W) at Centre Street (S). The driver door of MV3 was inaccessible. The collsion resulted in air bag deployment for MV2 and MV3. I observed moderate damage to MV1's front grill/bumper area. I observed substantial damage to the front and rear of MV2 and substantial damage to the front and driver side of MV3. Tody's responded and removed MV2 and MV3 from the roadway. Photos were taken of the scene and submitted to the IT Bureau. Officer Martin of the Watertown Police Department was requested by Sgt. Lee to respond to the scene and assist with a commercial truck inspection of MV1. Officer Martin contacted Trooper Grey of the Massachusetts State Police Truck Team who also responded to assist with the truck inspection. Sgt. Lee remained with Officer Martin and Trooper Grey during the inspection. MV1 was relocated to Richardson Street for the inspection. Trooper Grey determined the truck was in compliance with all required paperwork and it passed it's mechanical inspection. MV1 was removed from the roadway by a state tow. I spoke with the operator of MV1, William Tisbert (S24792655). Tisbert stated he was operating a 2006 KW W900 Semi Truck (NH SEMI: AP2638) Northbound on Centre Street. Tisbert stated as he was approaching Mt. Ida Street, a vehicle came out of no where and he hit it. Tisbert stated there was no one in front of him and then a vehicle came off of a side street (Mt. Ida Street) and attempted to turn left in front of him to return to the side street. Tisbert stated he could not avoid hitting the vehicle. Tisbert crashed into the rear of MV2. MV2 then crossed into the opposite lane and crashed into another vehicle. Tisbert remained on scene and reported no injuries. Tisbert stated he was wearing his seatbelt at the time of the accident and was not distracted. Tisbert later admitted to crashing into the rear of MV2. I spoke with the operator of MV2, Janet Vanbuskirk (S59829146), at Newton Wellesley Hospital. Vanbuskirk stated while operating her 2004 Toyota Highlander (MA: RS 529Y), she was stopped on Centre Street (N) at Mt. Ida Street. Vanbuskirk had her left directional on and was waiting for traffic in the Southbound lane to clear to take a left turn onto Mt. Ida Street. Vanbuskirk stated MV1 crashed into the rear of her vehicle. As a result of the impact, Vanbuskirk's vehicle traveled into the opposite lane and crashed into the driver side of MV3. Vanbuskirk reported minor injuries to her neck, left arm, and right hand. I spoke with the operator of MV3, Madeleine Karolian (S23979811), at Beth Israel Deaconess Medical Center in Boston. Karolian stated she was operating her 2016 Toyota Prius (MA: EV5746) Southbound on Centre Street. Karolian stated MV1 crashed into MV2, and MV2 crashed into her vehicle. Karolian stated she did not have time to avoid the collision. Karolian stated she had minor injuries to her neck and chest. Karolian's vehicle had a dash cam recording at the time of the accident. I viewed the footage with her through an app on a cell phone. The footage shows MV3 traveling Southbound on Centre Street on August 8, 2019 at 1308 hours. Ahead of MV3 in the Northbound lane, MV2 is seen stopped on Centre Street before Mt. Ida Street with it's left directional activated. I observed MV1 travel Northbound on Centre Street and crash into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane and crashed into the driver side of MV3. The footage then ends. A copy of the footage was emailed to my work email. The footage was then forwarded to IT Director Steve Smith to be added to the media for this report. As a result of my investigation, the operator of MV1, William Tisbert, will be cited for Newton City Ordinance Chapter 19, Section 71 (Following too closely). Massachusetts Uniform Citation T1270942 was completed and mailed to Tisbert's residence.

I spoke with the operator of Ma Reg 1XM680 Elizabeth CHISOLM who stated she was attempting to take a left onto Washington St, a vehicle in the right hand eastbound lane was turning right into the parking lot, CHISOLM stated she did not see Ma Reg EV3452 being operated by Julien GRANT in the left eastbound travel lane until it was too late colliding with him. I spoke with GRANT who stated he was traveling EB in the left travel lane of Washington St when struck by CHISOLM exiting the hospital.

I met with the operator of MV1, Newton Firefighter Keive Mullings. Mullings stated he was operating Newton Fire Ladder 2(MA MF4951) Eastbound on Beacon Street towards Centre Street. Mullings stated the rear passenger side bumper area of MV1 made contact with a unoccupied motor vehicle parked Eastbound on Beacon Street in a metered spot across from 837 Beacon Street. No injuries were reported by the occupants of Ladder 2, I observed minor damage to the rear passenger bumper area of MV1. I spoke with the owner of MV2, Pakizeh Motiee (2017 BMW X3, MA: 7NCC90). I observed minor damage to the front driver side quarter panel area of MV2. Motiee was advised. Pictures of both vehicles were taken and submitted to the IT Bureau.

MV2, (MA Reg: 5YR125; Honda CRV 2019) helped MV1 come to a stop because it was parked. Minor damage to MV1 front driver bumper and MV2 scratch on the rear bumper. MV2 owner, Lourdes Meira, exited her residence at 313 California St to witness MV1 pressed against her vehicle. Meira called 911 because she observed the driver of MV1 unconscious or asleep. Fire arrived on scene and gave two doses of Narcan to MV1 operator, Keith Harrington. Harrington gained consciousness and medics transported to NWH. Harrington admitted to injecting heroin today prior to this incident. Harrington to be summons for 90/24G-OUI Drugs, 2nd offense and 90/24E-Negligent Operation (MA Uniform Citation:T2079021). Citation given in hand at NWH by Ofc. Guarinio. Tody's towed MV1, towed motor vehicle form filled out. Used syringe from MV1 center console safely disposed of in N492. Small silver container with white substance confiscated from center console beside used needle. Container was filed into property and evidence to be tested. An immediate threat was filed and faxed to RMV.

Based on the witness statement of Yehor Shumerko, the operator of MV1 rolled down Eliot Street subsequently backing her vehicle into the foundation of 330 Eliot Street. Both tenants of the home then stated that they exited the home and when attempting to speak with operator she appeared distraught and begged them not to call the police before then leaving the scene without providing any information. Both occupants of the home were uninjured. Fire responded and determined that the home was safe for the time being. Wellesley PD attempted to locate the owner of the vehicle at her residence with negative results. Photographs of the scene were taken and submitted to IT to be attached to this report. Massachusetts Uniform Citation T2079291 was issued via mail to the operator of MV1 for 90-24, Leaving the scene of property damage. Approximately 1 hour later Officer Guarino located the operator of MV1 several streets away on Columbia Ave. She stated that she was smoked a cigarette when he vehicle began to roll backwards and struck the house. The operator of MV1 also stated that she did not believe there was any damage to the home which is why she left despite major damage to her vehicle. Todys responded to the scene and towed the vehicle.

On Wednesday, October 16, 2019, I, Patrolman Hatfield, was assigned to marked cruiser N 495 to Division 5 for the 1530 to 2330 hour shift. At approximately 1742 hours, FTO Guarino and I were dispatched to Commonwealth Ave at Cedar St for a motor vehicle accident with minor injuries. FTO Guarino and I arrived on scene and noticed two MVs with damage. I spoke with the RO/operator of MV2, Janet Haines (Janet). Janet stated she was not suffering from injuries. Janet said she was operating southbound on Cedar St crossing Commonwealth Ave when she had a collision with MV2. Janet stated prior to crossing Commonwealth Ave, an unknown MV stopped at the intersection to allow her to cross. Janet then stated she proceeded accross when she crashed into MV1, who drove around the stopped MV. Janet said her brakes did not work and she continued to drive forward and ended on top of the curb of Commonwealth Ave facing eastbound. MV2 suffered damage to the whole front bumper and wheel wells. FTO Guarino spoke with the operator of MV1, Madeleine Richard (Madeleine). Be advised both front and side driver airbags were deployed, and the front driver's side door was unable to be opened. I then spoke with Madeleine. Madeleine stated she was operating eastbound on Commonwealth Ave before being involved in the collision. Madeleine stated she drove around the MV infront of her that was attempting to take a left from Commonwealth Ave onto Cedar St, and she did not see MV2 prior to the accident. MV1 was struck on the driver's side by MV2, and ended facing westbound in the middle of Commonwealth Ave. EMTs and fire arrived on scene. Janet stated she did not need to be transported. Madeleine was transported to Newton Wellesley Hospital for an evaluation. Both MVs were towed by Todys.

I was dispatched to 854 Beacon St, Newton, MA for an overturned motor vehicle accident. I arrived on scene and saw Vehicle #1 (MA reg 9AS213) overturned on its driver's side with the female operator/registered owner trapped inside. Newton Fire arrived on scene and mechanically removed the operator out of the motor vehicle. Prior to where Vehicle #1 was overturned I observed Vehicle #2 (MA reg 615FF6) with rear bumper damage and a snapped rear axle. Vehicle #2 was unoccupied. Operator of Vehicle #1 was transported to Newton-Wellesley Hospital for treatment by Newton Medics. Both motor vehicles were towed by Tody's and inventory sheets were completed and filed. I spoke with the Operator of Vehicle #1 at Newton-Wellesley Hospital. Operator stated that she does not remember what happened before the crash and that she only remembers waking up in her car. I told her that her car was towed and a report is on file. Ofc Selig spoke with the registered owner of Vehicle #2 and notified her of the incident. Based on my investigation it appears that Vehicle #1 struck the rear of Vehicle #2 and the operator over steered causing the vehicle to overturn onto the driver's side.

On 11/21/2019 at approx 0740 hrs while assigned to 497 I responded to the intersection of Washington St and Beacon St for a report of a two car crash with entrapment. Upon arrival I observed NFD and Cataldo Medics assisting the driver of Ma Reg 5RA826 which was in the left EB lane of Washington St with the driver side of the vehicle caved in , the driver of the vehicle, Dorcus BIRUNGI was crying and shaking complaining of left side pain . NFD was working on opening the driver side door which was stuck from the collision. BIRUNGI related she was travelling WB on Washington St through the intersection of Washington and Beacon St when she was struck . The crash forced her car to spin sliding over the center island coming to final rest in the EB lane of Washington St. I was approached by Newton School Crossing Guard Regina GAYLE who stated she had witnessed the crash. GAYLE stated the Operator of the black Jeep Ma Reg 8VX486 had the green light, was well into his left turn from the EB laft lane of Washington St onto Beacon St when the Suburu "came flying " through the intersection. GAYLE stated she didnt know if the Suburu driver was trying to beat the light but she was "going so fast". I spoke with the operator of Ma Reg 8VX586 , nicholas MANCINI. MANCINI stated he was taking the left onto Beacon from EB Washington St with the green light when the other operator came over the rise of WB Washington St at a extremely high rate of speed . BIRUNGI transported to NWH by Cataldo, Vehicle towed by Todys.

The operator MV1 of stated that he was traveling Southbound on Walnut St attempting to turn left and travel Eastbound on Beacon St when MV 2 attempted to pass MV 1 and side swiped the right side of MV 1. The operator of MV 2 stated that MV 2 was attempting to take a left onto Beacon St and MV 1 attempted to illegally pass MV 2 on the left hand side causing a collision. Fire and EMS responded as well but there were no injuries sustained to either party. MV 2 was towed by Tody's due to it being disabled on a public way.

On Saturday, November 30th, 2019 at approximately 1736 hours I, Officer Newton and Officer Healy responded to Centre St near the Mass Pike exit ramp 17 for a motor vehicle accident. The operator of vehicle 1 stated she was driving northbound on Centre St when vehicle 2 tried to merge from the right lane and struck her vehicle. The operator of vehicle 2 stated she was coming off the Mass Pike exit 17 and she tried to merge over to the lane to her left and did not see vehicle 1 and struck the vehicle. Gedansky stated that the front passenger side of vehicle one struck the front driver's side of vehicle two. Vehicle one had no damage and Gedansky was not injured. Vehicle two had damage to it's driver's side mirror and driver's side front bumper. I took photos of vehicle two and forwarded the disk to the NPD's IT bureau. Sgt. McNeil, N495, and I responded to 474 Wodward St. for an auto pedestrian accident. Upon arrival the victim, Elizabeth Cantwell was laying in the street and receiving treatment from NFD and Cataldo. The operator of Ma Reg. 192HV0 was Margaret Meehan. Meehan was visibly upset and was hearing impaired, as a result Meehan was unable to communicate what happened in the accident. At this time we were able to contact Meehans daughter, Bernadette Conley 781-248-6222. Cantwell was transported to Beth Israel Hospital. Traffic Bureau was notified. I went to the BI hospital to get a statement from Cantwell. Cantwell states that her husband parked their vehicle across the street from Starbucks. Cantwell states that she stood next to her car to cross the street. Vehicle 1 came to a stop and Cantwell believed that the vehicle was going to let her cross the street (not in crosswalk). Cantwell states that when she went to cross the street vehicle 1 hit her, causing her to land on the hood and then rolled off onto the street. At this time Cantwell has a dislocated hip and head injury. I took photos of the scene. Meehans vehicle was parked in a legal spot and I transported her to NHQ where she was picked up by her daughter Bernadette Conley. I filled and immediate threat with the RMV and a copy of the report was attached with the form.

On 01/16/2020, while assigned to N494, I, Officer Conary, responded to a hit and run on Walnut Street at Otis Street. Operator of MV1 stated that she was traveling Northbound on Walnut Street when MV2 came out of Otis Street taking a left turn on Walnut Street. MV2 hit MV1. MV2 continued to drive Northbound on Walnut Street and did not make an attempts to stop. MV1 pulled over and notified Newton Police. I checked the area for MV2 with negative results. There is damage to MV1 left front bumper and left side. Operator of MV1 said there should be damage to MV2 right side. MV1 was able to be driven from the scene. Newton Dispatch contacted the Operator of MV2 and left a message. I went by the address at 484 Lowell Ave, but the owner was not home nor was the vehicle involved. I left a message with her neighbor to contact Newton Police, and I also left an additional voicemail. Later in the evening, I was able to speak to the Operator of MV2. Operator of MV2 stated she was traveling Eastbound on Otis Street and taking a left Northbound on Walnut Street. She said that the driver Southbound on Walnut Street let her out to turn, and made the left turn onto Walnut Street. Operator of MV2 stated that she did not know that she hit another vehicle. She said she could have hit a curb. At one point, she said "they hit me". She checked the damage on her car and found a scratch on the right side back bumper area. I confirmed all her information and updated the report as accordingly. Operator of MV2 said she has Traveler's Insurance, but RMV is showing Standard Fire. Operator of MV1 was also notified. No further incident to report.

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

The pole is located on the island between the eastbound/westbound lanes of Commonwealth Avenue. The pole was knocked down however it landed completely within the grassy island and is still intact. On 02/26/2020 I responded to the area of Eastbourne Rd and Ireland Rd for a motor vehicle crash involving a pedestrian. Eastbourne RD is a public way which runs from Ward St north bound intersecting with Ireland Rd on the far north end. Ireland RD is a public way which runs west to east. Both the east end and the west end of Ireland Rd are dead ends. The crash occured on Ireland Rd's east end near the intersection of Eastbourne Rd. The crash involved Vehicle 1 described as a 2016 Acura SUV color grey bearing MA passenger registration 3RB127. Vehicle 1 had a valid inspection sticker which expires on 08/30/2020. Vehicle 1 was operated by its owner identified as Barbara Ball. Ball has an active MA Operators License (S18727836). The involved pedestrian was identified as Bruce Terrio. Terrio is an employee of Feeney Brothers Utility Services. Feeney Brothers Utility Services is a contractor of National Grid Gas Company. Upon arrival I observed the following: The crash occurred in the daylight hours around 10:30am on Ireland Rd near the intersection of Eastbourne Rd. At the time of the crash the weather conditions were clear, 48 degrees Fahrenheit and overcast skies (weather.gov). The area where the crash occurred was an active construction site for Feeney Brothers Utility Services. The area of the crash is a dead end. The area is accessed from Ward St from either Eastbourne Rd or Coolidge Rd. The entrances of both Eastbourne Rd and Coolidge Rd had construction signs indicating utility work in the area. At the time of the crash, all roadways were open to vehicle traffic. On the eastbound side of Eastbourne Rd just prior to the intersection of Ireland Rd, I observed a large air compressor which was parked in the roadway facing south bound. No traffic cones were located near the air compressor. To the right of the air compressor were thick green bushes. A wooden fence was located to the right side of the green bushes. On the east side of Ireland Rd I observed a large excavated hole on the right side of the roadway. The hole was near the intersection of Eastbourne Rd and was located approximately three feet from the curb. I observed four traffic cones, two on the front side and two on the left side of the hole. Located on the front edge of the hole was a tire print. Vehicle 1 was located on the west end of Ireland Rd. The front passenger tire of Vehicle 1 had fresh dirt located on the sidewalls and the tire tread. Vehicle 1 appeared to have no visible damage. The pedestrian (Terrio) was being treated by EMS and Fire for injuries. Terrio was wearing brown pants and a green reflective safety vest. I spoke with the operator of Vehicle 1 Barbara Ball. Ball stated that she was operating Vehicle 1 northbound on Eastbourne Rd near the intersection of Ireland Rd. Ball stated that she made a right turn onto Ireland Rd and drove on to the edge of the excavated hole. Ball stated that her vehicle then made impact with Terrio who was in the hole. Ball stated that she immediately stopped, backed out of the hole and called 911. Ball stated that prior to making a right turn onto Ireland Rd, there were no traffic cones in the roadway or next to the hole. Ball stated that as she made the right turn onto Ireland Rd she did not observe Terrio or the hole until it was too late. Ball states that she resides at 80 Eastbourne Rd which is the residence located to the immediate right of the crash location. Ball states that she had been recently parking on Ireland Rd due to ongoing construction at her residence. Ball states that she was traveling at a slow speed prior to impact. At the time of the crash there were three other Feeney Brothers employees working at the site. These workers were identified as Steven Gamble, Mike Cabrinma and Gerard McComaskey. At the time of the crash all three workers were located further up Eastbourne Rd near the intersection of Coolidge Rd and did not observe the crash. At the time of the crash, Newton Police Sergeant Glenn Chisholm was working a paid detail for Feeney Brothers. Sergeant Chisholm stated that he did not observe the crash.Terrio was transported by Cataldo EMS for minor injuries. Several photos were taken of the scene. Photos were taken of the hole with traffic cones and without traffic cones. All photo's were submitted to the IT Bureau. After clearing the scene, I responded to St Elizabeth's Hospital in Brighton, MA to interview Terrio. Terrio stated that at the time of the crash he was standing in the hole on Ireland Rd performing gas utility work. Terrio stated that the front passenger side bumper of Vehicle 1 struck him in the right shoulder. Terrio stated that the impact caused him to lose his balance and fall down in the hole. Terrio states that after falling down he was able to climb out of the hole without assistance. Terrio stated that at the time of impact he was wearing a white hard hat which he thinks struck the pavement on the edge of the hole. Terrio states that at the time of the crash he was working alone in the hole. Terrio states that at the time of the crash traffic cones were located near the hole. Terrio stated that as a result of the impact he sustained minor injuries to his right shoulder and back. I then observed Terrio's hard hat which appeared to have a fresh scrape mark on the top. The hard hat also had broken interior straps. Terrio stated that the straps broke when he was struck by Vehicle 1. Photos were taken of the hard hat and were submitted to the IT Bureau. After reviewing all facts of this incident, Vehicle 1 appeared to have been traveling at a slow speed prior to the crash. This was evident due to the lack of tire skid marks prior to the tire print on the edge of the hole. It also appears that Ball's view of the hole and Terrio may have been obstructed by the parked air compressor on Eastbourne Rd. The thick green bushes and a wooden fence to the right of the air compressor also added to the obstructed view. The hole was also located in close proximity to the air compressor which reduced Vehicle 1's reaction time. Jeff Oliver of OSHA was also notified of this incident. On 3/8/2020 at approx 1313Hrs while assigned to 497 I responded to 101 Alban Rd for a report of an elderly female with advanced dementia was missing along with husbands Lincoln Town car (see report 20012388). While on scene I was advised the elderly female, Blanche MUCCIARONE had crashed the car near 2150 Washington St. Upon arrival I found MUCCIARONE in the driver seat of Ma Reg 62344 , the front of the vehicle had major front end damage and the front driver side airbag had deployed and the vehicle was still in gear with MUCCIARONE appearing unable to understand how to place it in park. MUCCIARONE was confused and unable to communicate what had occurred. While NFD and Cataldo worked with MUCCIARONE I spoke with the operator of Ma Reg 566ET1, Patricia HESSION who stated she was stopped at the red light with her husband John HESSION in the passenger seat she was struck from behind by MUCCIARONE causing her to crash into Ma Reg 682G94 being operated by Leonid FRIEDMAN who was also stopped for the red light. MUCCIARONE was transported to NWH by Cataldo, Patricia HESSION was complaining of neck pain and elected to have her husband drive her to NWH. On 3/16/20 I was dispatched to 127 Commonwealth Ave for a report of a MV(#1) crash into a fire hydrant. The operator left the scene and was shortly after identified as Mr Bai Lin Wu. Wu called NPD to report the crash once he arrived home. Ofc Ferguson took photos of the knocked over hydrant. City Water Dept was notified of the damage. I responded to Wu's residence. Wu stated he was travelling on the Commonwealth Ave W/B carriage road. Wu stated an unknown MV travelling behind him was trying to pass him on the left. Wu stated he turned to the right to allow the unknown MV to pass. Wu ran off the road to the right and struck the hydrant in front of #127 Comm Ave. Wu further stated he left because he was trying to get medication home as soon as possible to his ill wife. On Wednesday, May 6, 2020 while assigned to Traffic unit N525, I responded to the area of Harvard Street and Washington Park, Newton for a report of a construction vehicle that crashed into a City of Newton fire hydrant. The weather at the time of the crash was clear and sunny. The road surface was dry. Harvard Street and Washington Park are both public ways maintained by the City of Newton. I spoke with the operator of the vehicle involved in the crash, Michael Clavette (S36731553). Clavette is employed S and P Construction and was operating a 1998 Mack 700 (MA CON: S71054) while working as a sub contractor for Feeney Brothers on Washington Park. Clavette stated he was attempting to back down Washington Park (W) from Harvard Street. Clavette stated he pulled his vehicle forward to reposition and the front bumper area made contact with a fire hydrant in front of 82 Harvard Street. I observed minor damage to the front bumper area of MV1. I observed minor damage to the fire hydrant. Photos were taken of the damage and submitted to the IT Bureau. The City of Newton Water Department was notified.

Both Parker St and Hagen Rd are public ways in the City of Newton. Vehicle 1 was then rear ended by Vehicle 2 described as a 2018 Ford Van color white bearing MA Con Reg S35168 which was traveling southbound on Parker St. As a result of the impact Vehicle 1 sustained damage to the passenger side rear bumper and quarter panel. Vehicle 2 sustained damage to the drivers side front bumper and fender. The operator of Vehicle 1 identified as Chelsea Halsor stated that she was stopped on Parker St due to another uninvolved vehicle which was stopped in front of her vehicle due to a pedestrian crossing Parker St in a crosswalk. Halsor stated that while stopped she was struck from behind by Vehicle 2. Halsor stated that the force of the impact caused her to hit her head on the steering wheel. Fire and Medics responded to the scene and evaluated Halsor. Halsor was released with a patient refusal. The operator of Vehicle 2 identified as Christopher Benoit stated that he was traveling southbound on Parker St and struck Vehicle 1 which was stopped in traffic. Benoit stated that he tried to avoid the collision by swerving to the right but was unable to and as a result struck the passenger side rear of Vehicle 1. Benoit stated he was uninjured. No other witnesses on scene. Vehicle 1 was driven from the scene by Halsor. An RMV check of Vehicle 2 revealed that the registration was expired as of 12/31/2019. Tody's responded and removed the vehicle. A Towed MV Inventory Sheet was completed. An RMV check also revealed that Benoits MA Drivers License (MA DL# S9550481) was suspended. As a result Benoit was issued in hand MA Uniform Citation T1268483 and cited for Mgl 90/23 Operation of a Motor Vehicle After License Suspension, Mgl 90/9 Operating an Unregistered Motor Vehicle on a Public Way and City Ordinance Violation 19-71 Following to Close.

On 06/10/2020 at 1249hrs I responded to the intersection of Fairway Drive and Watertown Street for a two car motor vehicle crash involving a city of Newton Fire Department vehicle. Vehicle 1 described as a 2009 Ford Taurus Sedan (MA Municipal Fire Reg 64260) was backing southbound on Fairway Drive when it struck Vehicle 2 which was unoccupied and parked in the southbound lane of Fairway Drive. Due to the force of the impact Vehicle 1 sustained minor damage to the drivers side rear quarter panel and Vehicle 2 sustained minor damage to the drivers side rear quarter panel. The operator of Vehicle 1 stated he was backing up due to a large commercial motor vehicle trailer which was parked and obstructing traffic in the northbound lane of Fairway Drive. No injuries were reported on scene. No vehicle tows were required. Pictures were taken and submitted to the NPD IT Bureau Officer Marini N537 responded for the commerical motor vehicle trailer.

On 6/26/2020 at approx 1214hrs while assigned to 497 I responded to the area of Washington St at Quinobequin Rd for a report of a 2 car crash with air bag deployment. Upon arrival I observed Ma Reg 579AM3 a gray Toyt Rav 4 and Ma Reg 219w70 a red Toyota Corolla in the middle of the EB lanes of Washington St both with significant damage, front and side airbag deployment and fluids leaking. I ascertained the operators were not hurt and called for two tows and NFD for speedy dry. I spoke with the operator of the red Toyt Brendan OCONNOR who stated he was travelling EB on Washington St, had the green light, was proceeding through the intersection when he was struck by the operator of the Toyt Rav 4,Gabriella NUGENT. I spoke with NUGENT who stated she was attempting to take the left onto Quinobequin Rd from the WB lane of Washington St when she collided with OCONNOR. NUGENT acknowledged that although the light was green WB she did not have the left arrow to turn. Both cars towed by Todys, both Nugent and OCONNOR signed patient refusals with Cataldo ambulance.

On 07/15/2020, while assigned to N494, I, Officer Conary, responded to the intersection of Centre Street at Church Street for a motor vehicle accident. Upon arrival, I spoke with Operator of MV1 who stated that he was driving Southbound on Centre Street when the car in front of him stopped suddenly at the green signal. At this time, MV1 hit MV2 rear. Operator of MV1 said that he believes she was distracted with a hand held device. Operator of MV2 explained to me that she at the intersection of Centre Street and Church Street traveling Southbound. She got the green signal and she let the car traveling Northbound take a left turn onto Church Street before she continued through the intersection. At this time, MV2 was hit by MV1 from the rear. She stated she was not using her phone. It should be noted that the signal traveling Northbound on Centre is a green left signal to turn onto Church Street. After the green arrow, cars yield to oncoming traffic to turn. Fire and Medics arrived on scene and all parties involved signed patient refusals. MV1 was towed by AAA. MV1 had minor damage to the front bumper. MV2 was able to be driven from scene with minor damage to the rear bumper. No further incident to report.

MV2 sustained extensive damage. MV1 continued westbound on Watertown St and struck a cross walk sign and Newton Fire Department box which both in front of Coletti-Magni Park, and both were destroyed. MV1 hit and hopped the curb of Watertown St in front of Coletti-Magni Park, drove onto the side walk, and hit the fence of Coletti-Magni Park and damaged much of it. MV1 then came at rest on top of the Coletti-Magni Park fence and sustained extensive damage. MV1 operator stated he was not injured at this time and sign a patient refusal with Newton Medics. MV2 was unoccupied. Both MV1 and MV2 were towed by Todys.

On my arrival with NFD and Medics I observed one vehicle Maryland license plate 9DF562, 2016 Ford Escape in the middle of the intersection with heavy front end damage. The other vehicle Ma commercial V42208, 2014 white transit van resting into the chain link fence to the property of #99 Jackson Rd with heavy damage to the passenger side front quarter. I first spoke with the operator of the Van, identified as David Dwyer. The Van which David was operating is owned by Porcelain Patch & Glaze Corp. #140 Watertown St, Watertown Ma. David stated he wasn't sure what happened. He reported going N/B on Jackson Rd passing thru the intersection of Pearl St when the crash occurred. I asked him if he possibly might have not stopped at the traffic light(red)? He responded, "he wasn't sure". I then spoke with the other operator identified as Raul Davila. He reported that he was going W/B on Pearl St crossing the intersection of Jackson Rd when vehicle #2 struck his vehicle in the intersection. He further stated his light was definitely green. Both vehicles were towed from the scene by Todys. Both operators signed patient refusals. Mr Davila did complain of dizziness but refused to go with the Medics. Operator #2, David Dwyer was cited with Ma citation #T2014122 for failing to stop for a red light on Jackson Rd. The citation was later left at his work place at #140 Watertown St, Watertown Ma. with a fellow employee. David had gone back out on the road to a job. The property to which vehicle #2 caused damage to, I later gave the owner, Fernarla Thomasis the insurance information from that vehicle.

Vehicle 1 a city of Newton Fire Engine was parked in the driveway of Newton Wellesley Hospital 2014 Washington St. Vehicle 2 was located in the driveway directly behind Vehicle 1. Vehicle 2 collided with Vehicle 1 while attempting to pass Vehicle 1 on the left. As a result of the impact Vehicle 1 sustained minor damage to the drivers side. On August 13th, 2020, while working in marked unit N526, I responded to a 2 car MVA involving pedestrians at the intersection of Beacon Street and Chestnut Street, Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street and Chestnut Street are both public ways maintained by the City of Newton. The operator of MV1, Maria Dinapoli (S92626906), stated she was operating her 2018 Toyota Corolla ( MA REG: 1KNW71) heading west on Beacon Street and had the green light at the intersection of Chestnut Street. Dinapoli stated she was traveling straight through the intersection when MV2 which was traveling east on Beacon Street at the time cut infront of her vehicle to take a left turn onto Chestnut Street. Dinapoli stated she applied her brakes but was unable to stop in time and made contact with MV2. The impact of the crash caused MV1 to drive over the curb and onto the sidewalk where it made light contact with a baby stroller with a 1 year old inside. MV1 also struck and damaged a Newton Fire Department box that was located on the sidewalk. The baby stroller was being operated by the mother identified as Anna Gilman (S11328154). The pedestrians reported no injuries and signed patient refusals with Medic 2. MV1 sustained moderate front end damage and was towed from the scene by Todys. Dinapoli also reported no injuries and signed a patient refusal with Medic 2. The operator of MV2, Leena Dolabany (S12410136), stated she was operating her 2018 Jeep Grand Cherokee (MA REG: 1TCZ14) heading east on Beacon Street and had the green light at the intersection of Chestnut Street. Dolabany stated she was in a line of cars in traffic waiting to take the left turn onto Chestnut Street. Dolabany stated as the cars infront of her continued through the intersection she took a left onto Chestnut Street and as she was already into the turn she was struck by MV1. MV2 sustained moderate damage to the passenger side panel. Dolabany reported no injuries and signed a patient refusal by Medic 2. A query of all vehicles and parties involved through dispatch determined that MV2 was revoked for insurance. After speaking with the operator I was informed that MV2 had recently switched insurance agencies and Dolabany was able to provide paperwork from Geico Insurance stating the vehicle was insured. Due to the registry not displaying a valid registration, MV2 was towed from the scene by Todys. I advised Dolabany to contact her insurance company and advise them to update their information with the registry so she can retrieve the vehicle. Photographs of the scene and damage to city property were taken by Officer Wilson and submitted to the IT Bureau. On August 14th, 2020 I spoke with the operator of MV2, Leena Dolabany and her father George Dolabany in the front lobby of the police station. They provided paperwork from Geico Insurance that stated the vehicle was infact insured at the time of the accident and that there was an error made by the Registry of Motor Vehicles. Sgt. Devine and I advised them that we have to use the Registry's database for our investigation and because it still states the vehicle is revoked for insurance we can not release the license plates. Sgt. Devine advised them they could get another tow company to come and take custody of the vehicle but the plates will be seized until the registry shows an active status on the registration. I issued Mass uniform citation T2017328 in hand to Leena and advised her she will be summoned for a hearing for the following violations; Ch 90 section 34J Operating MV without insurance and Ch 90 section 9 Unregistered MV.

On Tuesday, September 1, 2020, while assigned to marked unit N496, I responded to the area of 780 Beacon Street, Newton for a report of a motor vehicle crash involving a bicyclist. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of the vehicle involved in the crash, Dina Hudson (S33833807). Ms. Hudson stated she was sitting in her 2015 white Jeep Grand Cherokee (MA: 727SZ7) that was parked (E) in a metered parking spot in front of 780 Beacon Street. Ms. Hudson stated traffic in the Eastbound lane of Beacon Street to her left was stopped. There was a red light ahead at the intersection and traffic had built up past where her vehicle was parked. Ms. Hudson stated she opened her driver side door while she was looking for her debit card to go to the bank. Ms. Hudson stated at this time a bicyclist was traveling at a high rate of speed around the stopped traffic towards the intersection and crashed into her driver side door. Ms. Hudson stated the operator of the bicycle then traveled further up the roadway and crashed behind a vehicle stopped in traffic. Ms. Hudson stated she was unsure if the bicyclist crashed into the stopped vehicle or if it had landed on the roadway just before it. Ms. Hudson stated the bicyclist then got up from the roadway and began yelling at her that the crash was her fault. I observed a few small fresh scrapes on the outside of the driver side door area. When I initially arrived at the crash location, I observed Officer Lazarakis checking on his well-being of the injured bicyclist, Marc Lind. Lind was observed laying on the sidewalk in front of 780 Beacon Street. After being evaluated by Newton Medics, I spoke with Mr. Lind. Mr. Lind stated he was traveling on his Trek bicycle Eastbound on Beacon Street towards Langley Road. Mr. Lind stated while traveling through the area at this time, traffic in the Eastbound Lane was stopped and there was a red light ahead of him at the intersection. Mr. Lind estimated his speed at this time to be 10-12 miles per hour. Mr. Lind stated as he travelled past the Santander Bank at 780 Beacon Street, the operator of a parked white Jeep to his right abruptly opened their door into traffic. Mr. Lind stated he attempted to avoid the vehicles door and veered to the left. Mr. Lind stated the right end of his handlebar made contact with the door and as he traveled forward and applied his brakes his right side crashed into the rear of a vehicle stopped in traffic ahead of him. Mr. Lind stated he crashed to the roadway on his right side. Mr. Lind was able to move to the side of the roadway and wait for Newton PD, Fire, and Medics to respond. Mr. Lind reported injuries to his right arm, side, and leg area. Mr. Lind signed a patient refusal with Newton Medics and was transported away from the area by his wife with his bicycle. I observed no substantial damage to his Trek bicycle. At the time of the crash, Mr. Lind was wearing a white t-shirt, black shorts, black, shoes, and a gray helmet with sunglasses. I spoke with a witness to the crash, Steve Worthington. Mr. Worthington stated he was stopped in traffic on Beacon Street about 4 vehicles back from the red light. Mr. Worthington stated he looked out of his passenger door mirror and observed a bicyclist "whipping" down Beacon Street in between vehicles parked on the side of the road and vehicles stopped in traffic at the red light. Mr. Worthington estimated the speed of the bicycle to be approximately 20 miles per hour. Mr. Worthington stated the bicyclist had to "lean" by his vehicle to get by. Mr. Worthington stated a vehicle just ahead of him that was parked opened their door and the bicyclist made contact with the door. Mr. Worthington stated the bicyclist then went down in front of the car that was in front of him. Mr. Worthington stated the bicyclist was rolling around on the ground and screaming "It's your fault" at the woman that opened her door Photos were taken of the bicycle and the vehicle involved and submitted to the IT Bureau.

On Friday, September 18th, 2020 at approximately 1619 hours I, Officer Newton, was dispatched to Brookline St and Spaulding Ln(both are public ways in the City of Newton) for a report of a vehicle that crossed over Brookline St and struck a tree and drove off with major front end damage. The RP was able to give a description of the vehicle as a White Audi with the license plate beginning with 8X. The RP also stated the vehicle was going down Dedham St and possibly turned down Marvin Ln. Sgt M. Wade located the vehicle, White Audi MA Reg 8XVP10, with major front end damage, parked outside of 45 Indian Ridge Rd and made contact with the operator, John Bigda. It should be noted that the house is 2 houses from Marvin Ln. Upon arrival I spoke to John. John stated he was driving his vehicle and got into an accident but it was not a big deal because he was able to drive home. John was not able to explain the details of his crash and was very evasive as to exactly what happened, just that he got into an accident because somebody cut him off. John continued to tell me multiple times nothing bad happened and everything was fine. John was very unsteady on his feet and was stumbling around and almost fell several times while I was trying to speak with him. John also had slurred speech and a strong odor of an alcoholic beverage coming from his person that I was able to smell even though I was wearing a mask. At this time I asked John to take field sobriety tests to which he refused by saying "I'm not taking the test." The Medics arrived on scene to evaluate John. John was not able to answer their questions and the medics decided he should go to the hospital for an evaluation. However, John refused to go to the hospital. NFD arrived on scene and stated they saw tire marks and debris at the location of the crash. Ofc. Guarino went to that location to take pictures that will be added to this report. Due to the above facts and because John was almost falling over while speaking with us, he was placed under arrest for Operating Under the Influence of Liquor. John was handcuffed (D/L) and searched prior to being placed in cruiser 499. Cruiser 499 was searched before and after transport for contraband with negative results. John was booked and read his Miranda rights by Sgt M. Wade and Ofc Donovan processed him. John was handed citation T2014721 for 89/4/A Marked Lanes Violation (Driving over the double yellow line and leaving the roadway to strike the tree on Brookline St), 90/24/C Leaving the Scene of Property Damage (Hitting a city tree and causing damage), and 90/24/K (Operating Under the Influence of Liquor, 2nd Offense.. 1st offense was 11/18/2004 OUI docket 0443CR2527A CWOF ). The citation was placed with John's property. The vehicle was towed by Todys.

On Monday September 21st, 2020 while in N526 I responded to Newton Fire Station #2 located at 1750 Commonwealth Ave for a past accident involving Newton Fire Ladder #1 and a Cataldo Ambulance. The accident is reported to have occurred on Thursday September 17th 2020 at 10:00am. At the time of the accident the weather was partly cloudy and the road surface was dry. Captain Roche stated he was working at the station that day when a Cataldo Ambulance truck MA AMR:AMB41 slowly backed up and struck the top of the ladder on the Ladder Truck#1 MA MVN: MF463. Ladder #1 was parked and unoccupied at the time in front of the Fire Station. At the time no damage was reported on either apparatus. Today damage was noticed on the top part of the ladder. Pictures were taken of a crack and some damage to the ladder. I spoke with the operator of the ambulance identified as EMT James Perry (S14785886 ) who stated on Thursday at around 10:00am he was delivering spare parts to another ambulance and was slowly backing up in front of the Fire Station. James stated he was going about 2MPH when he bumped into the front of Ladder #1. James stated there was no damage reported to either apparatus at the time. This minor accident occurred on City of Newton Property in the driveway of Fire Station #2. No injuries were reported. Pictures of Ladder #1 were submitted to the IT Bureau.

On 09/30/2020 I responded to the area of Waverley Ave and Brackett Rd for a single vehicle motor vehicle crash involving a utility pole. Both Waverley Ave and Brackett Rd are public ways in the City of Newton. Vehicle 1 described as a 2011 Kenworth commercial box truck (RI Con Reg 80859). Vehicle 1 was traveling southbound on Waverley Ave when it struck overhanging utility wires (Eversource Pole #44) located over the roadway near the intersection of Brackett Rd. As a result of the impact Vehicle 1 sustained major damage to the roof of the cargo area. The force of the impact caused the utility pole #44 to break in half. It should be noted that at the time of the crash the weather was steady rain with high winds. No injuries were reported on scene. The operator of Vehicle 1 stated that he was traveling southbound on Waverly Ave and did not see the power lines until it was too late. After further investigation it was determined that a tree(private) located on Brackett Rd had come down due to the high winds and landed on power lines. These power lines were also attached to utility pole #44. The force caused the power lines from utility pole #44 to sag over Waverley Ave. As result Vehicle 1 did not have enough clearance under the power lines. Vehicle 1 was towed from the scene by Tody's Towing. Towed MV inventory completed. Due to the live wires in the roadway, Newton Fire responded and secured the scene. Eversource was also notified and responded to the scene. Pictures were taken and submitted to the IT Bureau.

On Wednesday October 14, 2020 at approx. 0901 hours I responded to the area of 277 Elliot Street for a report of a two motor vehicle crash unknown injuries. Upon arrival on scene, Newton Fire personnel and medics were on scene for an evaluation. Both parties refused / advised treatment and signed patient refusals. I spoke with the oper of v1, she stated a vehicle in front of her stopped abruptly and she slammed on her brakes and was stopped when she felt impact from vehicle 2. Oper v2 stated that vehicle 1 stopped abruptly, he slammed on his brakes and due to the roadway being wet his vehicle slid into v1. He stated he had some irritation from the front air bag deploying. Witness stated a vehicle in front of him stopped abruptly to turn left, which caused him to stop abruptly, which caused chain reaction.

On November 1st, 2020 at approximately 14:02 hours while assigned to N491 I responded along with Newton Fire and Medic One to the intersection of Pearl St @ Jackson Rd for a MV crash with minor injuries. On my arrival I first observed in the intersection a green colored Volkswagon with Vermont tag,HPK270 with extensive damage. The other involved vehicle a white Jeep Wrangler, bearing Ma tag, 8WK846 was located on the property of the Lincoln Elliot School, 191 Pearl St, also with extensive damage. The operator of the green Volkswagon was identified as Viorel Marin. He stated he was going E/B on Pearl St and was approaching the intersection of Jackson Rd when the light had turned green. As he was entered the intersection vehicle #2 came from his right side(N/B) on Jackson Rd at, according to him, a high rate of speed causing his vehicle to collide with the Jeep. He doesn't remember much after that. I observed a small cut to his right knee and later he showed me a small abrasion to his forehead. All his air bags had deployed on contact with vehicle #2 . I located the operator of the Jeep who was sitting on the ground being assisted by an unidentified women(not a passenger). She appeared shaken but had no visible signs of any serious injury from the crash. She identified herself as Tara Feeley. According to Ms Feeley she had been travelling N/B on Jackson Rd and was attempting to continue straight across the intersection of Pearl St when she was struck by vehicle #1 in the intersection. The impact caused her vehicle to flip over unknown amount of times finally landing on the property to the Lincoln Eliot School. Based on operator #2's statements, my observations of the scene, damage to her MV, it appeared her vehicle after contact with vehicle #1, flipped over N/B across the intersection first hitting the traffic light on the northeast corner, additionally making contact with the utility pole just east of the traffic light before finally landing right side up on the property of the Lincoln Eliot School. She reported climbing out her passenger side window when her vehicle finally stopped. She said,"she was lucky she had her seat belt on". I agreed. Ms Feeley also reported that she was sure that her light was green as she entered the intersection. I attempted to locate any possible witnesses from the crowd that were gathered near the crash site. There were two that heard the crash but none that actually saw what happened. I couldn't locate any cameras near the intersection. Both vehicles were towed from the scene due to the extensive damage/both totaled. Both operators had minor injuries and both declined to be transported to the hospital. They signed patient refusals. Officer Danny Anderson later took several pictures of the crash scene and the all the property damage. Both utility companies were notified. The camera disc was placed in the technology bureau mailbox.

On 11/11/2020 I, Officer Mildner, was dispatched to 48 Windermere Rd. for a report of Gas Lamp being struck by a single MV. Operator of MV1 stated that he was traveling Northbound on Windermere Rd when he took a left and pulled into the driveway of 47 Windermere Rd. in an attempt to back out and turn around. Operator of MV1 stated that as he was backing out Eastbound, he hit a pole containing a gas lamp at the top, right in front of 48 Windermere Rd. Witness 1 stated he saw Operator of MV1 pull into the driveway of 47 Windermere and back out directly into a pole containing a gas lamp at the top. There were no injuries. No damage to MV1. Operator of MV1 states he just left Wegman's where he works the overnight shift and was traveling home to 223 River Street. Operator of MV1 states he was traveling westbound on Commonwealth Ave and was going to take a right hand turn onto Lowell Ave when he doesn't know what happened and struck the concrete street light pole dislodging it from its base where it laid across MV1 and part of the roadway. It should be noted while speaking with the operator of MV1 there was a slight language barrier and he couldn't elaborate much on the events leading up to the crash. MV1 sustained heavy front end damage and air bag deployment. Medics were requested for an evaluation and found no signs of fatigue, impairment or injuries and cleared with a signed patient refusal. Coviello Electric was notified for the street light pole and the Newton Fire Department capped off the exposed wires from the base and it was taped off with caution tape. Tody's towing responded and removed the vehicle from the roadway. The operator of MV1 insisted on walking home from the scene.

On, 11/19/2020 at approximately 1700 hrs, Newton Police and Fire were dispatched to Commonwealth Ave. and Prince St. for a reported head on motor vehicle collision with injuries. Upon arrival, I observed MV#1 ( 1WDR15) facing eastbound in the westbound travel lane. MV#2 (986KW1) was also facing eastbound, but in the eastbound travel lane. Both vehicles had significant front end damage, consistent with a head on motor vehicle collision. The operator of MV#1 was sitting on the grassy median upon my arrival. She stated she was traveling eastbound on Commonwealth Ave. when she became distracted by the radio, crossed over the double yellow line into the westbound lane of Commonwealth Ave. and collided with MV #2. The operator of MV#2 was entrapped in her vehicle, but was able to be extracted from the vehicle by Newton Fire personnel. Operator #2 was able to inform me that she was traveling westbound on Commonwealth Ave. She noticed MV#1 cross over the double yellow line at a high rate of speed, and collide into her vehicle. The operator of MV#2 was conscious and alert and transported to Tufts Medical Center Boston for an evaluation. The operator of MV#1 declined medical attention and was given a Massachusetts Uniform Citation (T2014484) for violating MGL. Ch. 89. Sec. 4(a) Marked Lanes Violation. Both vehicles were towed away from the scene by Tody's Towing Service. Photographs of this collision were taken on the (N492) camera and will be attached to this report.

On 12/3/2020 at 1400hrs I investigated a past motor vehicle crash involving a pedestrian. The crash occurred on 12/03/2020 at approximately 1240hrs. The crash was located in the area of 483 Centre St. Centre St is a public way in the city of Newton. Vehicle 1 was traveling southbound on Centre St near the area of 483 Centre St. Pedestrian 1 was located on the southbound side of Centre St near 483 Centre St. Pedestrian entered the southbound lane of Centre St and was struck by Vehicle 1. As a result of the impact Pedestrian 1 sustained a minor injury to his left leg. Vehicle 1 had no known damage. No parties were transported. No tow was required. It should be noted that the crash was reported by the Operator of Vehicle 1 Daniel Zandman at approximately 1400hrs. Zandman states that he was operating his Vehicle southbound on Centre St just after the intersection at Church St. Zandman states that Pedestrian 1 unexpectedly ran in front of his vehicle. Zandman stated that he did not have enough reaction time to come to a complete stop and his front bumper made contact with Pedestrian 1. Zandman stated that after impact he immediately got out of his vehicle and checked on Pedestrian 1. Zandman described pedestrian on as a 10 year old white male identified as Dexter Jagoditish. Zandman stated that while on scene Dexter called his mother Jess Jagoditish who immediately responded to the scene. Zandman stated that he exchanged information with Jess Jagoditish who then took Dexter home. Zandman states that a witness Mia Martin was also on scene. At approximately 1425hrs I called and spoke to Dexter and his mother Jess Jagoditish. Dexter states that he was walking on the sidewalk near 483 Centre St. Dexter states that he wanted to cross Centre St and thought there were no cars coming. Dexter stated that he entered the southbound lane of Centre St and a car came out of no where and hit him. I then spoke to Jess Jagoditish who is Dexter's mother. Jess stated that she received a call from Dexter around 1240hrs stating he was just hit by a car. Jess stated that when she arrived on scene Dexter was with Zandman and a witness Mia Martin. Jess stated that both Zandman and Martin explained them what had happened. Jess stated that Dexter appeared to be ok so she decided to take him home. Jess states that at this time Dexter has a bruise on his left calf but otherwise appears to be ok. At 1551hrs I spoke with the witness Mia Martin. Martin states that just prior to the crash she was walking her dog behind Dexter. Martin states that "out of nowhere" Dexter sprinted into the southbound lane of Centre St. Mia states that she then observed Vehicle 1 attempt to slow down and swerve to the lest to avoid Dexter. Mia states that the passenger side front bumper then made contact with Dexter. Mia states at impact Vehicle 1 was traveling at a very slow speed. Mia states that Dexter was then knocked on to the ground but then immediately jumped up and started to run away. Mia states that both she and Zandman were able to get Dexter to comeback so that they could check to see if he was ok. Mia states that Dexter was then able to contact his mom who responded on scene. Due to this crash being reported after the fact, Fire, EMS or Police where not initially notified and did not respond to the scene. Pictures were also not taken of the crash scene. At this time this report is for documentation purposes. After speaking with all the parties involved it appears that the crash was caused by Pedestrian 1 entering the roadway in a manner which gave Vehicle 1 little or no time to react.

On 12/05/2020 at approximately 22:00 hours I was called in to respond to a single vehicle crash with serious injuries in the area of 291 Nahanton St. Nahanton St is a pubic way in the City of Newton. Upon my arrival at the crash scene with Sgt Devine I was briefed by Officers Daly and Anderson, ( see Ofc Daly's report for further information CAD # 20054728 in regard to changes take out against the operator of the truck ) I made the following observations of the crash scene, Nahanton St is a public way in the City of Newton that is a two way roadway divided by an unprotected merdium ( double yellow line ) that runs east to west. The roadway surface was wet due snow and ice. The roadway was plowed and the snow was pushed to the shoulder, althought wet the roadway was icy. There was approximately 2 inches of snow on the shoulders of the roadway. The road was light with overhead head street lights. The posted speed limit is 25 MPH. I observed a 2013 Chevrolet Silverado pick up truck that had major front end damage. The Chevrolet was at an angle with it's front end facing west bound and resting on the grass shoulder and into a wooded area. The Chevrolet rear was sticking out into the shoulder of the east bound lane of Nahanton St. The passenger side front and rear doors of the Chevrolet had been removed by the fire department to extricate the driver and passenger. The driver and passenger side air bags were deployed . The front passenger side windshield of the Chevrolet was completely missing and the driver's side had completely shattered. The front tires, hood, and engine had been compacted on it's self. The front and rear driver's side doors had been mechanically open by the fire department. The speedometer needle had stopped at 54 MPH. The Chevrolet left the roadway and left tire marks in the snow in the eastbound bike lane of Nahanton St. At a 106.4 feet from the beginning of the tire marks the first point of impact was with two tree stumps that were 3.5 feet from the roadway, four feet tall and 3.5 feet in diameter. The tire marks continue in the east bound bike lane and exit the roadway and onto a dirt shoulder at 136.1 feet from the beginning of the tire marks. The truck continued to the right of a utility pole and then it's passenger side made contact with a tree at 170.8 feet from the begin of the tire marks. The tree was 12 feet from the bike lane approximately 5 feet in diameter. The impact with the tree caused the truck to turn so that the rear was facing north east on the dirt shoulder and bike lane. The final point of rest of the rear passenger side tire was 200 feet from the beginning of the tire marks. The truck's rear diver side tire final point of rest was 209 feet from the beginning of the tire marks. The front driver's side tire was compacted into the trucks engine and the passenger side front tire was located behind the tree approximately 20 feet from the roadway. Sgt Devine took photos of the crash area and of the truck and the disk was download by the NPD's IT Bureau. The truck was towed from the crash scene by Todys's Towing to their lot for storage, I fill out a towed motor vehicle form. There were no reported witness who observed the crash. The crash was reported by an unknown passerby. On 12/07/2020 Ofc Gaudet faxed a request for preservation of medical records, blood and urine samples of Schneider to Beth Israel Deconess Medical Center.

On 12/8/20 I was working N492 when I responded to Crafts St. at Albermale Rd. for a report of a 2 car MVA with minor injuries. Upon arrival I spoke with the Operator of Veh 1 Megan Stekla. Stekla stated she was traveling Eastbound on Crafts St. when a Vehicle pulled out from Albermale Rd. to cross Crafts St. causing the car in front of her (Veh 2) to slam on the breaks, causing her to collide with Veh 2. I then spoke with the occupants of Veh 2 Brandon Alvarado and Sofia Alvarado. Brandon stated he was traveling Eastbound on Crafts St. when a vehicle attempted to cross over Crafts onto Albermale Rd. almost causing him to strike that vehicle. Brandon stated he slammed on his breaks to avoid the collision and was struck from behind by Veh 1. Sofia stated she had recently gotten a concussion and did not feel well. Fire and medics responded and Sofia was transported to NWH. The vehicles had minor damage and were able to be driven from the scene. Negative results on any description of the vehicle that cut across Crafts St. Both operators were provided a crash number.

On Monday, December 21, 2020, while assigned to Traffic unit N525, I responded to 8 Cragmore Road, Newton for a report of a motor vehicle crash involving a City of Newton Fire apparatus. Cragmore Road is a public way maintained by the City of Newton. The weather at the time of the crash was cloudy. The road surface was wet from melting snow in the area. The operator of MV1, Jon Papia, stated he was operating City of Newton Fire Engine 7 (MA MVN: MF470 ) Southbound on Cragmore Road. As MV1 travelled past 8 Cragmore Road, it's passenger side made contact with the front driver side area of MV2 (MA: 1JTC19). The Honda Accord was parked and unoccupied at the time of the crash. The registered owner was made aware of the crash and advised. I observed no damage to the passenger side of MV1. I observed damage to the front driver side of MV2. Photos were taken and submitted to the IT Bureau. No injuries were reported by any of the involved parties.

On 12/28/2020 at approx 1159hrs while assigned to 497 I responded to the area of Tamworth Rd for a report of a tractor trailer pulling down wires and utility poles. Upon arrival I observed the intersection of Radcliff Rd and Tamworth Rd blocked by a tractor trailer truck with multiple downed wires and broken telephone poles. I spoke with the operator of the tractor trailer, Lawrence TOGBA who stated he was following the detour off of Chestnut st to Quinobequinn Rd as he has repeatedly in the last several weeks when, this time , likely due to recent storms, the wires were hanging lower getting caught in his truck pulling and causing telephone polls 702/6 and pole 7 on Tamworth Rd to snap and telephone pole 1 on Radcliff Rd to snap. NFD, Eversource and Verizon responded and were able to free the truck from the wires and begin repairs to infrastructure. 526 Traffic Officer Wilson responded and took photos

MV1 stated she was attempting to manuever around vehicles to exit the street when "I stepped on the gas and not the brake" while in reverse subsequently going backwards, over a curb, across the lawn, and into the bushes of 17 St James Ter sustaining heavy damage throughout the vehicle. On January 22, 2021 at approximately 08:17 hours I responded to a single motor vehicle crash in the area of Commonwealth Ave at Grant Ave. It was reported that a motor vehicle struck a tree and a fire hydrant. Upon arrival I spoke with MV1, who said that she was traveling southbound on Grant Ave when she saw a vehicle traveling northbound traveling close to the solid double line. MV 1 got nervous and swerved to the right causing her to jump the curb and strike a city tree. MV1 then reversed into a fire hydrant, causing it to become dislodged. Traffic Officer Ciccone responded and took pictures of the damages and Tody's removed the vehicle from the scene.

The operator stated; due to the snowy, wet, slippery conditions, her vehicle slid into the concrete median. On Tuesday February 9th, 2021 at approximately 1334 hours I responded to the intersection of Ascentra Terrace and Highland Street for a two car motor vehicle accident. At the time of the accident the weather condition was heavy snow in the area and the roads were covered in snow. Both Ascentra Terrace and Highland Street are owned and maintained by the City of Newton. Upon arrival I met with the operator of MV1 identified as Elizabeth Carron (S53478024). She stated she was heading northbound on Highland Street when she lost control of her vehicle on the bend of the road and made contact with MV2. MV1 sustained heavy rear end damage and airbag deployment. The operator of MV2 identified as David Balaban (S09328852) stated he was traveling southbound on Highland Street and could not avoid MV1 when it lost control and collided with his vehicle. MV2 sustained heavy drivers side panel damage. The Fire department and Medic 2 arrived on scene and cleared with a patient refusal. Todys responded and towed MV1 as it was inoperable.

MV#1 was traveling north on Woodchester Rd, attempting to make a left turn onto the west bound side of Mandalay Rd. Due to freezing conditions and light rain, both of these roads were covered in a layer of ice. When MV#1 attempted to make the left turn, the vehicle slipped around the corner and struck a fire hydrant at the corner of above mentioned roads. City of Newton water department was called out to inspect the hydrant, and N522 arrived on scene and took pictures. Passenger in MV#1 was transported to NWH for possible neck injury. MV#1 was driven off scene by operator.

The operator of MV#1 stated that he was traveling north on Cherry St when he struck the fire hydrant in front of #35 Cherry St. Upon further questioning, the operator Johnathon Benson stated that he has been diagnosed with a medical condition that sometimes affects his coordination. I asked if he should be driving due to his medical condition and he stated his doctor has suggested he not drive. I then asked if he thought it was his medical condition or another factor which caused the crash. Mr. Benson stated he believes his medical condition is likely what caused the crash. When Mr. Benson exited his MV I noticed a large wet spot on both the front and rear of his pants around the groin/buttocks area. I then asked Mr. Benson if he has had siezures in the past, to which he stated he has. I then asked Mr. Benson if its possible that he may have had a seizure while driving today and he stated it was. Mr. Benson did not sustain any injuries, his MV was towed by Tody's and the Newton Fire Dept. was notified. At this time due to the information Mr. Benson provided and my concern about his/others safety while he operates a MV, I am filing an Immediate Threat with the RMV. On 3/8/21 I mailed citation #T2080897 to Johnathon Benson for operating Tx. Reg. HVY7642, which was unregistered at the time of the crash.

Vehicle 1 described as city of Newton Fire Engine #4 (MA Fire Reg 444) backed into Vehicle 2 which was parked and unoccupied facing northbound on Armory St. As a result of the impact Vehicle 1 sustained little to no visible damage, Vehicle 2 sustained minor damage to the passenger side front bumper. No injuries were reported on scene. No tows were required. Photographs were taken and submitted to the NPD IT Bureau.

On 3/22/2021 at approx 1134hrs while assigned to 497 I responded to the intersection of a two car crash with unknown injuries. Upon arrival I observed both vehicles, Ma Reg 8GP319 a gray VW Passat and Ma Con Reg T67757 a white Ford F350 work truck interlocked on the southwest corner surrounded by fluids and debris from the crash, all airbags in the Passat were deployed and the front right axle of the F350 appeared to be broken. Both operators were complaining of minor pain, the passenger of the Ford had cuts and scrapes and was complaining of a possible broken rib. NFD and Fallon ambulance and Todys arrived and attended to the fluids, injured and disabled vehicles. I spoke with a witness, Tony Abbiati who stated he was behind the Passat at the light on Chestnut St SB, he stated they had the green light , the Passat was traveling through when he observed the Ford not stopping for the red light colliding with the Passat forcing into the south east curb of the intersection. the operator of the Passat, Anthony Nastasti stated he was travelling SB on Chestnut St through the intersection with the green light when struck by the ford. I spoke with both the operator Thomas Marzano and the passenger of the Ford, Michael Devincent, both stated they did not remember what happened. Nastasti and Marzano both signed patient refusals, Devincent transported to NWH by Fallon. Both vehicles towed by Todys.

On Thursday May 13th, 2021 at approximately 1353 hours myself and several units responded 55 Chapel Street for a motor vehicle that struck a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. Chapel Street is a public way owned and maintained by the City of Newton. Upon arrival Fire and medical personnel were attempting to remove the pedestrian from underneath the vehicle as her right leg was stuck in the undercarriage. Fire and medics were able to successfully free the pedestrian from the vehicle and render medical attention. The pedestrian was identified as Beth Herr ( S58524067) and it was determined that she was also the owner of the vehicle involved. I spoke to witness Joe Shields who stated he was outside his house located at 56 Chapel Street when his attention was brought to a blue SUV that was unoccupied moving backwards down Chapel Street. Joe stated he first witnessed the operator of the motor vehicle pull up to the UPS drop box located on the sidewalk across the street from his house. He stated he saw the female operator step out of the vehicle and drop off a package. Next he saw the vehicle start to roll backwards and the female operator opened the drivers side door and attempted to pull the emergency break. He stated the vehicle overtook her and she fell to the ground with her foot stuck under the car. Joe states he heard her yell and saw her get dragged by the vehicle to the opposite side of the street where the vehicle was stopped by the curb. Joe stated he tried to assist the woman but she was yelling in pain and he did not want to cause more injury so he waited for help to arrive. Witness Robyn Preston stated she was walking on Chapel Street when she saw the blue SUV rolling backwards with no one inside the vehicle. She states she then saw a woman try to stop the vehicle but she got dragged underneath the vehicle to its final resting place. Robyn stated she stayed on scene until help arrived. I was able to speak with Beth inside the ambulance prior to being transported to the hospital. Beth was conscious and alert but in a considerable amount of pain and discomfort. I asked her what happened to which she stated she drove her blue Toyota Rav4 MA REG: 1FHM53 to the UPS drop box located in the area of 55 Chapel Street. She stated she thought she put the vehicle in park but must not have as it started to roll backwards when she got out of the car. She states she tried to stop the car by opening the drivers side door and attempting to pull the emergency break. The car over took her and she was dragged to the ground with her right foot stuck underneath the vehicle. She was dragged across the street where the vehicle was finally stopped by a curb. Beth sustained minor scrapes and road rash on various parts of her body and her right leg was injured. Fallon medics transported her to the Brigham & Womens Hospital to be treated. Her husband was contacted using Beth's cell phone and notified of the accident. Captain Boudreau and I were able to drive her vehicle to her residence at 50 Broadway Street, Newton. Her vehicle was left parked outside her house and her car keys were left with her husband who was inside the residence. The vehicle did not sustain any damage. Pictures of the scene were taken and submitted to the IT Bureau. I will be following up with Beth at the hospital to check on her status. On Friday May 14th at 0900 hours I spoke with Beth on the phone. She stated she is home from the hospital and doing much better. She states she has right leg pain and road rash from the accident.

Operator of vehicle one Ziting Liu stated on 05/14/2021 at approximately 17:38 hours she was driving west bound on Commonwealth Ave in her vehicle a 2015 Honda CRV ( MA reg 497yy1 ). Liu stated that she was driving into the setting sun and was wearing sunglasses at the time. Liu stated that as she approached the four way intersection with Chestnut St she observed that west and east bound traffic on Commonwealth Ave had a green light and that north and south bound traffic on Chestnut St had a red light. Liu slowed her vehicle to turn left onto the south bound lane of Chestnut St. It should be noted that both Commonwealth Ave and Chestnut St are both public ways in the City of Newton. Liu stated that she did not observed any " cars " driving east bound on Commonwealth Ave so she proceed to make the left turn onto Chestnut St. Liu stated that she realized that her vehicle has struck an unknown object as she was turning left. Liu stopped her vehicle and saw that her vehicle had struck a female bicyclist. Liu stated that the bicyclist was thrown from their bike and was on the ground. Liu stated that she was not injured as a result of the crash. Liu's vehicle had damage to the front passenger side hood . The front passenger side of Liu's windshield had an inward crack that was approximately 12 inches in length and 12 inches in width. The type of inward crack on the windshield is indicative of an outside force making contact with it. Liu's vehicle was towed by Tody's Services to their lot for storage. I filled out a towed motor vehicle form. Bicyclist Isabella Turinovic stated that she was ridding her bike , east bound on commonwealth Ave near Chestnut St. Turinovic stated that east bound traffic had a green light. Turinovic stated that as she was crossing over Chestnut St Liu's vehicle struck her causing her to hit the windshield. Turinovic was tossed from her bike and onto the roadway. Turinovic was wearing a helmet at the time of the crash. Turinovic stated that she did not lose consciousness and she was transported to Newton Wellesley Hospital via EMS from treatment . Turinovic agreed to have her bike taken by NFD Ladder One to be stored at NFD station Two. I took photo's of Liu's vehicle, Turinovic's bike and the disk was downloaded by the NPD IT Bureau. Witnees Alexander Delima stated that he was the first vehicle stopped a red light on Chestnut St in the south bound lane. Delima stated that Liu's vehicle did not yield to Turinovic as she turned left onto the south bound lane of Chestnut St from the west bound lane of Commonwealth Ave .A second witness Lindsay Bentis stated that she was the second vehicle stopped a the red light in the south bound lane of Chestnut St behind Delima. Bentis stated that she also saw Liu fail to yield to Turinovic as she turned onto Chestnut St. Based upon statements made to be by all involved parties, I concluded that Liu is at fault for this crash. I issued via U.S. Mail, Liu MA Uniform Citation T1446670 and cited her for a violation of MGL 90/14, failure to yield to oncoming bicyclist when turning left.

A grey older model trailblazer backed into utility pole 1/18, causing a power surge at 425 Watertown Street. Fire responded to the building for smoke inside, when they arrived the witness told them what had happened. Eversource was notified. There were no other witnesses. The vehicle involved had damage to the rear passenger side tail light. Unknown registration. Description provided was a grey older model chevy trailblazer. At approximately 2000hrs the owner of the vehicle contacted NPD. The operator stated he was backing out of the parking spot and struck the untility pole. He stated he observed his rear passenger side tail light cracked and did not know there was damage to any wires or the utility pole. He proceeded eastbound on Watertown Street. The owner of the building/property where the accident occured contacted NPD. I spoke to Mr. Franchi and provided him the accident number for insurance purposes.

On Thursday July 01, 2021 at 1413 hours while on patrol in N496 I responded to 1280 Centre St for a report of a motor vehicle that had crashed into the front stairs of the building. On arrival I saw vehicle 1 ( MA reg#271RM5) half way up the front concrete steps of the building and wedged on an iron fence. The operator was identified as Norma Greenberg and she was already out of the vehicle and stated she was uninjured. Greenberg stated she was in her vehicle parked in a parking spot attempting to pull out and her vehicle would not go forward. Greenberg stated she kept hitting the gas when the vehicle suddenly took off and went up the stairs. Greenberg reported no prior mechanical defects with the vehicle. It appears she was in neutral and then tapped it into drive as she pressed the accelerator. Newton Fire had to use a saw to take apart the iron railing due to it being wedged into the vehicle. The vehicle was towed from the scene by Todys. The area was roped off with caution tape due to the damaged railing and I notified the building manager, Jodie Zussman of the report number. Greenberg was transported home by N494 Officer Lazarakis.

On Friday, July 16, 2021, while assigned to Traffic unit N525, I responded to the intersection of Commonwealth Avenue and Temple Street, Newton for a crash involving a City if Newton Fire Department vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. Commonwealth Avenue and Temple Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Mr. Jacob Forbes. Mr. Forbes stated he was at a stop sign on Temple Street and was attempting to cross Commonwealth Avenue (N) from Temple Street. Mr. Forbes stated he looked to his right, and then to his left and did not see any vehicle approaching. Mr. Forbes stated as the began to enter the intersection he looked to his right and saw a vehicle pulling over to the side of the road and a Newton Fire truck with it's lights on was heading towards him. Mr. Forbes stated he could not avoid the collision and the front end of MV2 crashed into his passenger side. Mr. Forbes reported no injuries and signed a patient refusal with Fallon Medics. I observed damage to the front passenger side area of MV1 and it was removed from the roadway by Tody's Towing. MV2 is a City of Newton Fire Department vehicle "C2". It is a 2019 Ford Expedition (MF455). The vehicle was responding to a Fire Alarm Activation call with emergency lights and siren activate at the time of the crash. The occupants of the vehicle were taken to Newton Wellesley Hospital for evaluation. I observed front end and driver side damage to MV2. There was air bag deployment inside the vehicle, on both the front driver and passenger side. Tody's Towing removed MV2 from the roadway. Photos were taken of the crash scene and submitted to the IT Bureau. I responded to Newton Wellesley Hospital and spoke with Newton Firefighter John Morris (Driver) and Deputy Chief Michael Mcnamara (passenger) in the emergency room. Both parties stated they were responding to a call with emergency lights and siren activated. FF Morris stated as he approached the intersection of Commonwealth Avenue (W) and Temple Street, he observed a black SUV exiting Temple Street onto Commonwealth Avenue. FF Morris stated as they were passing he believed MV1 tried to speed up to cross Commonwealth Avenue. MV2 then crashed into MV1. As a result of the crash, FF Morris suffered right hand/wrist injury. Both FF Morris and Deputy Chief Mcnamara stated they had slight respiratory issues from breathing in the powder from the air bag deployment. MV1 failed to yield to MV2's right of way as he attempted to cross Commonwealth Avenue from Temple Street. The operator of MV1, Mr. Jacob Forbes will be mailed Massachusetts Uniform Citation 071113AB for Chapter 89, Section 8 (Fail to Yield at Intersection).

Operator of vehicle one Newton FF Eric Howard stated that while getting into the drivers seat of Newton Medic Unit 3 a Fallon Ambulance ( 2021 Ford Econoline MA reg AMN 7715 ), that was parked in front of 19 Faxon St, he inadvertently stepped on the break and disengaged the gear shift so that the ambulance went from park to drive. FF Howard was getting ready to drive Newton Medic Unit 3 to Newton Wellesley Hospital while Fallon Medics attend to their patient in the rear of the ambulance. FF Howard stated that after the ambulance went into drive it drove south bound on Faxon St and it's front driver's side fender struck the rear passenger side bumper of vehicle two. FF Howard stated that he was not injured in the crash and was able to drive the ambulance to Newton Wellesley Hospital. Fallon Medics Jared Gaglio and Shane McMahon stated that they were in the rear of the ambulance treating their patient, Dominica Tramontozzi when they felt the ambulance move forward and strike vehicle two. Both Gaglio and McMahon stated that they were not injured in the crash. I was unable to take a statement from Tramontozzi. Operator of Vehicle two Newton FF Joseph Colella stated that he was sitting in the driver's sea of Newton Fire Dept Eng 8 ( 2004 E-One Pumper MA reg MF471 and Newton Fire LT Douglas Quinn was sitting in the front passenger seat when the crash occurred. Vehicle two was parked in front of 19 Faxon St. FF Colella stated that the front driver's fender of vehicle one stuck the rear passenger bumper of vehicle two. Both FF Colella and Fire LT Quinn stated that they were not injured in the crash. Vehicle two did not require a tow. Vehicle one was towed from Newton Wellesley Hospital after a tow was arraigned by a Fallon Ambulance Supervisor. I took photos of both vehicles and forwarded the disk to the NPD's IT Bureau for downloading. Faxon St is public way in the City of Newton.

On 7/27/2021 at approx 0939hrs while assigned to 497 I responded to the intersection of Fenwick Rd and Tamworth Rd for a report of a two car crash with possible injuries. Upon arrival I observed Ma Reg 1CNR59 being operated by Svertlana Savenco pinned between the southern curb of Tamworth facing Eastbound and Ma Reg 62EZ69 being operated by Renata Pawska. I spoke with Renata who stated she had been travelling NB on Fenwick Rd intending to turn right onto Tamworth when she saw Tamworth east of Fenwick was shut down due to a detour, at that time Renata turned left colliding with Svetlana. I spoke with Svetlana who stated she was travelling straight ahead when Renata turned into her. Both vehicles towed by Todys, NFD put down speedy dry for fluids, Renata signed a refusal with Fallon Ambulance, Svetlana was transported by Fallon complaining of leg and back pain.

On 07/27/2021, while assigned to N494, I, Officer Conary, spoke with DEALE, Cathy (Operator of MV1) over the phone about a past hit and run. DEALE explained to me that she parked Westbound on Washington Street in front of 513 by the fire hydrant. Between 1200-1400 MV1 was hit on the driver side, and the mirror broke off. Operator of MV1 did not see any other vehicle standing by, she left the area and notified Newton Police. Operator of MV1 sent me pictures to confirm the damage. I went back to the area of where the accident was reported and confirmed glass and a pieces of a mirror on the side of the road.

On August 7th, 2021 at approximately 08:40 hours while working N491, I along with NFD and Fallon ambulance responded to #266 Nevada St for a report of a MV crash involving a pedestrian. On my arrival the fire and medics were speaking with the pedestrian that was struck. He identified himself as SEAN DAZET, DOB 04/3/81, #28 Ashmont Ave, Newton. He stated while walking on the sidewalk with his dog S/B on Nevada St he observed a vehicle backing out a driveway to #266 Nevada St. He said the vehicle was probably ten feet away from him when he yelled out to the driver but the vehicle was already on him. Mr DAZET unable to retreat out of the way put up his left hand to brace for the impact. Mr DAZET was then struck from the vehicles passenger side rear bumper. He was knocked back but never fell to the ground. Mr DAZET reported the vehicle backing out was going he estimated between 5/10 miles per hour prior to the crash. Mr DAZET reported to the medics he was experiencing left hip and left shoulder pain. However he signed a patient refusal. I spoke with the operator of involved vehicle identified as Patrick Alvarez. He stated while he was backing out from the driveway to #266 Nevada St his vehicle made contact with the pedestrian. He further stated he never saw the person walking past the driveway as he was backing up until he heard him yelling out. He immediately stopped his vehicle. The accident was a low impact crash. Mr DAZET was given the operator's information along with his insurance in case he had to seek further treatment for his non visible injuries. Finally Mr DAZET did indicate he probably will follow up with his primary care doctor on Monday. There was no damage to the involved vehicle.

Narrative:
At approximately 1640HRs on Thursday, August 12, 2021, I came across a motor vehicle crash at The intersection of Grant ave. and Commonwealth Ave.'s Carriage Lane. MV 1 (MA Pass: 882WWK) was facing SB on Grant ave., in the intersection. I observed it to have extensive front end damage (bumper, hood, driver's side head light and a pushed in radiator core support) and it was leaking fluids. MV 2 (MA Pass:8RS517) was facing WB in the Carriage Lane and displayed damage to its front passenger's side ( hood, head light, bumper and fender). Upon speaking to the operator of MV1, they stated that they were traveling straight (SB) on Grant Ave. and had a green traffic light. As they approached the intersection, MV 2 made a left hand turn and struck MV 1. Upon speaking to the operator of MV 2, they stated they were traveling on Grant Ave. (NB) and attempted to make a left hand turn onto the Carriage Lane (WB). It was at this time they struck MV1 because they did not see them. Upon speaking to witness 1, they stated they were traveling behind MV1 and that the traffic light was indeed green. They then observed MV 2 traveling at a high rate of speed, as if they were attempting to "cut off" MV1. When MV 2 attempted to make a left hand turn, they impacted into MV1. All parties involved stated that they were not injured and did not wish to been seen by EMS. Newton Fire arrived on scene and cleaned up the fluids. MV1 was removed by Todys service and MV2 was able to drive away under its own power. I gave the operator of MV 1 a ride to NPD HQ to await their ride and then cleared.
