Narrative:
On 01/03/19 at 07:26 hours, I responded to a two car motor vehicle accident at Boylston Street and Ellis Street with one reported minor injury (headache). Operator of MV # 1 states she was exiting the Boylston Street (RT 9EB) off ramp at Ellis Street when she states that she came to a stop for the stop sign. She then inched out and thought it was clear to proceed straight through and struck MV # 2 on both passenger side doors. Operator also stated that she did not think traffic would be allowed to come from the left of her. She was advised that there is a sign below the stop sign that states that the oncoming traffic does not stop. Operator was issued MA Uniform Citation T1441046 for Ch. 89/9 Stop Sign Violation. MV # 1 sustained front end damage to the entire front but was still driveable and the operator reported no injuries. Operator of MV # 2 states she was operating straight through the Boylston Street overpass (SB) on to Ellis Street when she states that MV # 1 never stopped for the stop sign. MV # 1 then proceeded out and struck her vehicle on the passenger side. MV # 2 sustained moderate damage to both passenger side doors and was towed by Tody's towing. The operator complained of a headache but signed a patient refusal with Cataldo Ambulance One.

Operator of MV1 stated he was traveling westbound on Commonwealth Ave. when MV2 pulled onto Commonwealth Ave from Chestnut Terrace onto Comm. Ave. As MV2 was pulling onto Comm. Ave the vehicle struck the driver side front and rear door. The Operator of MV2 stated she was taking a right onto Comm. Ave from Chestnut Terrace and hit MV1 with the front driver side bumper. MV1 had moderate damage to the driver side front and rear door. MV2 had minor damage to the driver side front bumper. Both operators and passengers did not suffer any injuries and both vehicles did not need to be towed. Operator of MV2 was cited for Ch. 90/10 Unlicensed Operation (T1440967), see incident #19000357. The owner of MV2 arrived on scene and drove the vehicle home.

On 01/04/19 I responded outside of #54 Needham St for a two vehicle crash. Veh #1 was a delivery box van who was driving due North on Needham St. The operator of veh #1 stated he was attempting to take a left into #55 Needham St. when veh #2 came from behind him on his left and he struck veh #2 as he turned. The operator of veh # 1 was taken by Ambulance #1 to Newton-Wellesley Hospital for an evaluation after complaining of pain in his chest area. The passenger of veh #1 stated everything happened "so fast" he didn't see what transpired. Moderate damage to veh #1. The operator of veh #2 stated he was traveling due North on Needham St. and was behind veh #1 for a long stretch of the road. The operator of veh # 2 stated veh #1 put his right directional on three different times but didn't turn. He stated veh #1 had his right directional on and was in the right lane when he went to pass him. Veh #1 then turned into his rear passenger side as he passed. Moderate damage to veh # 2. The operator of veh #2 stated the operator of veh #1 was walking around and yelling about the accident for a period of time and then eventually grabbed his chest and fell to the ground complaining of pain. From the result of my investigation it appears as if veh # 1 attempted to turn from the wrong lane and veh # 2 was passing in the "suicide lane", also wrong lane. There will be no citations issued at this time as both operators were given verbal warnings.

On 01/15/2019, at 1919 hrs, I responded to the intersection of Washington St and Centre St for a two car MVA. Upon arrival, I observed heavy damage to the front end of MV1 and moderate damage to the rear end of MV2. The operator of MV2 complained of head/neck injuries and was transported to Newton Wellesley Hospital by Cataldo for an evaluation. The operator of MV1 stated she was attempting to merge onto Washington St from Centre St when she rear ended MV2. The operator of MV2 stated when she slowed to merge onto Washington St from Centre St she was rear ended by MV1. Both vehicles were towed by Tody's. Towed MV inventory forms were filled out and filed.

Vehicle #1 stated she was stopped at the stop sign at the intersection of Goddard St. and Winchester St. when she had the right of way, she proceeded to drive through the intersection and was struck in the left driver side door by Vehicle #2. Vehicle #2 stated she believed she had stopped at the stop sign and entered the intersection when Vehicle #1 drove in front of her and she was unable to stop, striking Vehicle #1 in the side with the front of her vehicle. Vehicle #1 had moderate damage to the front left corner and driver side, however it was able to be driven away and did so. Vehicle #2 had moderate damage to the front of the vehicle and was unable to be driven and towed by Tody's Towing. Both drivers were evaluated on scene by medics and stated they were uninjured and declined any further medical treatment.

Operator of MV1 stated while traveling westbound on Commonwealth Ave. as he was approaching the red light at the intersection of Commonwealth Ave and Grant Ave. MV2 side swiped his rear right bumper. Operator of MV2 stated while traveling westbound on Commonwealth Ave. as he was slowing down to stop at the red light on Commonwealth Ave. and Grant ave. his vehicle began to slide. Operator of MV2 attempted to avoid MV1 by steering his vehicle to the right but side swiped the rear right bumper of MV1. After hitting MV1, MV2 struck a tree. MV1 had minor damage to the rear right bumper and was able to drive away. MV2 had moderate damage to the front bumper and hood. MV2 was able to drive the vehicle away. Both operators and passengers stated they were not injured.

On 1/19/19 I responded to 25 Normandy Rd for a report of a past hit and run. I spoke with the Operator of MV#1 who stated the accident had taken place in front of Tom's Pizza, 423 Lexington St, at approximately 1830 hrs. Operator of MV#1 stated she parked her vehicle in a parking spot directly in front of the restaurant and went inside. When she returned approximately 5 minutes later she realized her vehicle had been rear ended. Two witnesses provided her with information about the accident and the plate number. (See witness narratives.) MV#1 suffered moderate damage to the rear and was driven from the scene. Waltham PD responded to the address listed for MV#2 and were not able to locate the vehicle. At approximately 2000 hrs, I was contacted at the police station by the owner of MV#2's wife. After further investigation (see incident report #19002425) it was determined that the operator of MV#2 at the time of the accident was the vehicle owner's brother. I spoke with Operator of MV#2 on the phone and he stated that the road was icy and slippery due to snow and he crashed into the back of MV#1. He stated he was nervous and thought he had only hit the sidewalk curb. Operator of MV#2 was issued citation T1440626 by mail for 90/24C Leaving the Scene of Property Damage, 90/10A Unlicensed Operation and 90/24 Use of MV Without Authority. He stated the vehicle suffered minor damage to the front.

Vehicle #1 stated she was traveling south bound on Grant Ave. Vehicle #1 stated that in the area where Grant Ave. bends to the left the roadway is dark and she had trouble seeing the road. Vehicle #1 stated at the last second she saw the road bending left and attempted to turn but skid on the snow and struck a small boulder in the front lawn of 173 Grant Ave. moving the rock a few feet. Vehicle #1 then skidded and came to a stop in the middle of the lawn of 173 Grant Ave. Vehicle # 1 then pulled off the lawn and to the side of the road. The owner of the property of 173 Grant Ave. was also present and was notified of the accident. Vehicle #1 had moderate damage to the front of the vehicle but was still able to be driven and was driven home. Both the operator and passenger in Vehicle #1 stated they were uninjured in the accident and refused any medical treatment.

On 01/22/19 I responded to a three vehicle crash on Needham St. in front on #241- Marshalls Plaza. The operator of veh #1 stated she was driving due West on Needham St. when she looked right at a car waiting to pull out of the parking lot. When she looked back to Needham St., the traffic was stopped and she hit her brakes and snow slid covered her windshield. Operator of veh #1 never saw the impact, but veh #1 struck stopped veh #2 which was pushed into stopped veh #3. Moderate damage to veh #1 and 2 but no damage at all to veh #3. Operator of veh #2 said she was going "2-3 MPH " when she was pushed into veh #3. Operator of veh #3 claimed she was dizzy from the accident and signed a patient refusal from Ambulance #1. Engine #7 was also on scene assisting. Operator of veh #1 was cited in hand Mass citation #T1441211 for Ch 90/ Sec 13- failure to remove snow from vehicle . All vehicles were driven away from the scene.

THERE WAS ALSO A WITNESS TO THE HIT AND RUN ACCIDENT IDENTIFIED AS RYAN WALSH WHO STATED HE WAS TRAVELING SOUTHBOUND ON CRAFTS ST WHEN MV 2 FAILED TO STOP AT THE STOP SIGN, CAUSING HIM TO SWERVE OUT OF THE WAY TO AVOID A COLLISION. WALSH ALSO STATED THAT THE VEHICLE WHO BLEW THE STOP SIGN AND CAUSED THE ACCIDENT WAS A COMCAST VEHICLE. I CONTACTED COMCAST SUPERVISOR BRIAN DELANO (617 756 6293) WHO WORKS OUT OF THE NATICK OFFICE AND IS IN CHARGE OF EMPLOYEES IN NEWTON. HE STATED HE LOOKED AT GPS RECORDS AND STATED ONLY ONE OF HIS EMPLOYEES WAS IN THE AREA AT THE TIME OF THE CRASH AND HE WAS STATIONARY AT ADDRESS ON MELVILLE AVE. I ALSO CALLED JOHN MCGUINNESS WHO IS THE SUPERVISOR FOR COMCAST WORKING IN WESTON, WALTHAM AND WATERTOWN AREAS. I LEFT A VOICEMAIL FOR HIM TO CALL ME BACK. I RECEIVED A CALL BACK FROM JOHN MCGUINNESS WHO RELAYED TO ME THAT HE SENT A MESSAGE OUT TO ALL OF HIS EMPLOYEES INQUIRING ABOUT THE ACCIDENT IN QUESTION AND ONE OF HIS DRIVERS CALLED HIM AND TOLD HIM THAT HE WAS THE PERSON INVOLVED IN THE ACCIDENT. I MET WITH HIM WHO WAS IDENTIFIED AS CAMDEN DUQUETTE AND HE STATED THAT HE STOPPED AT THE STOP SIGN, PROCEEDED FORWARD AND DIDN'T SEE A TRUCK COMING ON HIS LEFT. HE STATED HE WAS ABLE TO AVOID THE FIRST CAR AND DIDN'T REALIZE HE STRUCK ANOTHER VEHICLE (STATED HE THOUGHT HE STRUCK THE CURB).THERE WAS DAMAGE ON HIS PASSENGER SIDE CONSISTENT WITH STRIKING A VEHICLE AND NOT THE CURB. AS A RESULT OF THE INVESTIGATION HE WAS ISSUED MA UNIFORM CITATION T1441118 FOR CH .90 S. 24 LEAVING SCENE OF AN ACCIDENT (PROPERTY DAMAGE) AND CH. 89 S. 9 FTC STOP SIGN.

On 2/9/19 I responded to 54 Ash St and met with Ms. Zardar Hovhannisyan in regards to a past hit & run to her vehicle. Hovhannisyan parked MA reg 1EC474 on Ash St (a northbound public one way street) just prior to Commonwealth Ave around 0800 hrs this morning. When she returned around 1000 hrs, she noticed moderate damage to the front left area of the vehicle. There was minor debris on the ground consistent with the damage I viewed. It should be noted that there are parking stalls parallel to where the vehicle was parked, with several residents of Norumbega Gardens coming and going quite frequently. It is not believed that there are cameras facing the specific area where the collision occurred. Hovhannisyan was advised to contact her insurance company.

On Wednesday, February 13, 2019 while assigned to Traffic unit N525, I responded to the 280 Newtonville Avenue, Newton for a report of a past hit and run accident. I met with Madeleine Aubourg (S67566148) who stated she is a Nursing Assistant at Cabot Park Village. Aubourg stated due to the snowfall the area had the previous day, Aubourg left her vehicle parked (S) in the rear parking lot overnight. Aubourg stated this morning when she returned to work she observed damage to the front bumper area of her 2018 Toyota Camry (MA: 6AN179). Aubourg stated she believes sometime overnight when the parking lot was being plowed a plow truck hit the front of her vehicle. I observed moderate damage to the front bumper area. I spoke with Adolfo Merida-Diaz (S44133458) on scene who stated he is employed by Marquense Landscaping and is responsible for the snow removal at 280 Newtonville Avenue. Merida-Diaz stated last evening and this morning he removed snow from the parking lot and did not make contact with any vehicles in the parking lot during his duties. I showed Merida-Diaz where the alleged accident occurred. Merida-Diaz stated he does not remember a car being parked in that particular spot, but if there was a car parked there he leaves enough room so he is not near any vehicles. I did not observe any surveillance cameras in this portion of the parking lot. There are no witnesses to the alleged accident. Aubourg was advised to speak with her insurance company.

On 3/1/19 while working N493, myself and N492 Officer Selig were sent to the area of 1354 Washington St for a motor vehicle accident with reported injuries. On arrival, I spoke to Matthew Digregorio DOB7/2/74 ( hereby referred to as operator #1) who was standing on the sidewalk next to MA reg 134TR9 (vehicle #1 ). Vehicle #1 had significant damage to the front and right quarter panels. It was straddled between the travel lanes and did not have the keys in the ignition. It was immediately apparent that operator #1 suffered an injury to his head, as there was a moderate amount of blood and small pieces of glass stuck in his forehead area. I observed operator #1 to be unsteady on his feet, have bloodshot eyes and slurred speech. I could detect a moderate odor of alchoholic beverage coming from operator #1 as I was approaching him, which was apparent from about an arms length away (it should be noted that medics were already en route to the scene at this time). Prior to the medics arrival, I asked operator #1 for a brief synopsis of what transpired. Operator #1 stated that he had just left Paddy's Pub and was driving westbound on Hunter St (which is approximately 1/8 mile from our location) when he collided head on with vehicle #2. Vehicle #2 was not immediately visible upon police arrival, however was pointed out as being located at the intersection of Hunter and Putnam St. I asked operator #1 where he was heading, as Hunter St is a back road not directly in the vicinity of Paddy's. Operator #1 could not answer the question and also remained silent when I asked him how much alcohol he had consumed this evening. At this time the medics arrived on scene. I asked operator #1 for the car keys to reliquish to the tow truck operator, which is standard operating practice. Operator #1 reached into his pocket and then provided me with the keys. Due to concern over operator #1's obvious head trauma, he was then transported to NWH to be evaluated while the investigation continued on scene. Two other parties approached myself and Officer Selig. One was the operator of vehicle #2, MA reg 1RD999. Operator #2 is identified as Luke Ferdinands DOB 5/11/80. The other party who approached is identified as Douglas Dasilva-Pimenta DOB 6/2/86 (Dasilva-Pimenta will now be referred to as operator #3). Operator #2 states that he was taking a right onto Hunter St from Putnam St, when he observed vehicle #1 on the wrong side of the road approaching him head on. Operator #2 states vehicle #1 then collided head on with him, took a right onto Putnam St, and continued onto the Washington St overpass near 1354 Washington St. Operator #3 states he was inside of #5 Hunter St when he heard a loud crash coming from outside. He looked out the window and saw vehicle #1 traveling at a high rate of speed on Hunter St towards Putnam St. Operator #3 observed damage to the right side of his parked vehicle (now referred to as vehicle #3) MA reg 7CAK80. He immediately got in his vehicle and proceeded to follow vehicle #1, which then crashed into vehicle #2 head on. Operator #1 did not stop until it was obvious that operator #3 was following close behind. This ultimately landed vehicle #1 at it's final location near 1354 Washington St. Vehicle #1 and vehicle #2 were not driveable and were towed to Tody's. It should be noted that there was a passenger in vehicle #2 at the time of the collision. Airbags were deployed and medics checked out both operator and passenger. Both declined transport. The passenger is identified as Elizabeth Ferdinands DOB 5/22/53.Ferdinands' version of events coincide with that of operator #2. Upon speaking with Officer Selig it was learned that operator #1 dropped his cell phone several times as he was holding it on scene. Both Officer Selig and operators #2 and #3 observed this. Operators #2 and #3 both expressed concern over the obvious odor of alcohol eminating from operator #1, and the fact that he was unsteady on his feet. I responded to NWH to follow up with operator #1 however he did not wish to speak further. He did express concern over the well being of all parties involved. Operator #1 was issued in hand the following citations: T1268426 - 90/24 OUI Alcohol, 90/24 Leaving the Scene (property damage, vehicle #2), and 90/24 Leaving the Scene (property damage, vehicle #3). T1268425 - 89/4a Marked Lanes violation. T1269216 - 90/24 Negligent Operation of a M/V (mailed after consultation with superiors) All streets mentioned are public ways in the City of Newton. 1354 Washington St will be the address used on this report and subsequent citations due to multiple locations being involved. Citations and paperwork have been placed in the prosecutor's mailbox. An immediate threat form has been filled out and faxed to Mass DOT. *Attention Police Prosecutor for blood work subpoena, if applicable*

MV#2 was parked on the side of the street and the operator contacted a personal tow for further assistance.

On Wednesday, April 10, 2019 while assigned to Traffic unit N525, I responded to the intersection of Centre Street and Wesley Street, Newton for a report of a motor vehicle accident involving a bicycle. It was also reported that the parties involved were arguing with each other. The weather at the time of the accident was cloudy and the road surface was dry. Centre Street and Wesley Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Abdulaziz Stewart (S04075169). Stewart stated he was operating his 2017 Toyota Camry (MA: 669FM3) at the time of the accident. Stewart stated he came to a stop on Wesley Street (E) at Centre Street and was attempting to take a left turn onto Centre Street. Stewart stated there were no vehicles in the Southbound lane of Centre Street and there was a vehicle stopped in the Northbound lane of Centre Street motioning to him to take his left turn. Stewart stated as he began to start his left turn, a bicyclist crashed into the front passenger area of his vehicle. Stewart stated the bicycle had been traveling Northbound on Centre Street in the Southbound Lane. Stewart stated after the collision the bicyclist was yelling at him and called him a "Nigger". Stewart and his occupant, Miriam Stewart, reported no injuries. Miriam is a student at Bigelow Middle School. I observed minor damage to the front passenger quarter panel area of MV1. I spoke with the bicyclist involved in the accident, Timothy Murtagh (S49233464). Murtagh stated he was traveling Northbound on Centre Street on his Cannondale bicycle in the Southbound lane. Murtagh was not wearing any safety equipment while operating his bicycle (helmet) and was wearing a dark red jacket. Murtagh stated as he approached Wesley Street, MV1 "jammed on the gas" and hit him in the roadway. Murtagh stated he then fell to the ground and landed in the middle of the street. While speaking with Murtagh I told him it was reported to me by the operator of MV1 that he used a racial slur during their interaction. Murtagh acknowledged that he did and stated, "I called him a nigger". I observed moderate damage to the front wheel/rim of Murtagh's bicycle. Murtagh reported no injuries. Murtagh was advised and issued Massachusetts Uniform Citation T1443441 for Chapter 85, Section 11B (Fail to ride on the correct side of the roadway). Photos were taken at the scene and submitted to the IT Bureau. Due to the language used by Murtagh during his exchange with Stewart, Officer Rosenbaum of the Community Services Bureau was made aware of the incident for follow up.

The weather at the time of the accident was clear and sunny. The road surface was dry. Washington Street is a public way maintained by the City of Newton. The operator of MV1, Jacqueline Dupre (S29256883), stated she was parked in front of 1302 Washington Street (E) in her 2018 Honda CRV (MA: 5054RF). Dupre stated she was attempting to exit her street parking spot and did not see any vehicles traveling towards her as she entered Washington Street. Dupre stated MV2's passenger side hit her front driver side area as she was attempting to enter the lane. I observed minor damage to the front driver side quarter panel/headlight area. Dupre reported no injuries at this time. The operator of MV2, Donnell Stoute (S53698210), stated he was operating a 2017 Porsche Macan (MA: 7ZC447) East on Washington Street. Stoute stated as he was traveling by 1302 Washington Street, MV1 pulled out of her parking spot and hit the passenger side of the vehicle he was operating. I observed moderate damage to the passenger side of MV2. Stoute reported no injuries at this time.

There are no witnesses or suspects at this time. There are surveillance cameras in front of Splash. I asked the employees in Splash if I could access the camera's, and they explained that they would have to wait until tomorrow to speak with the manager. No one was injured, and vehicle 1 did not need to be towed. On Monday, April 29th 2019, at approximately 1600 hours, I left a voicemail for the owner of Splash, Jamie Mcmanus, in regards to any possible surveillance footage. I requested that he call Newton Police headquarters and ask for me, when he gets the message.

Mrs. Weiss stated that she was attempting to back out of her driveway at 285 Crafts Street when her vehicle accelerated for no reason "like a bat out of hell." The vehicle then crossed Crafts street at a high rate of speed, jumped the curb, crossed the large grass island, and only stopped when the vehicle crashed into a tree. Based on the facts that Mrs. Weiss could not explain the reason for the cars acceleration, the distance the vehicle traveled without stopping, and the close proximity of the crash to an elementary and middle school I filed a request for Immediate Threat License Suspension/Revocation. All proper paperwork as well as a copy of this report were faxed to the Registry of Motor Vehicles.

On April 24, 2019, while assigned to N496, I was called by Dispatch to respond to the area Beacon Street and Hammond Street for a motor vehicle crash. This was called in by Boston College Police Department. Upon arrival MV #1 was on Beacon Street just past Hammond Street at the Island with considerable vehicle damage. MV#1 stated while traveling westbound on Beacon St, while approaching the intersection of Hammond he stated he started to feel strange and not like himself. MV#1 stated he then struck the curb with his front right tire, not believing his accident was very serious at this time. He reversed his vehicle off the sidewalk and proceeded to travel westbound on Beacon St. After passing through the intersection he stated his car felt very off at which point he initiated his hazards and pulled over to the right. MV#1 stated he has a history of epilepsy and currently takes Oxcarbazepine twice a day for his condition. He stated he did not believe he had a seizure or lost consciousness but did not recall the incident very well. Boston College Ofc. Cadogan witnessed the incident, and stated the following, while conducting a directed patrol at the intersection of Beacon and Hammond St he saw MV#1 traveling westbound on Beacon St. Ofc. Cadogan stated MV#1 then took a sudden tight turn on to the curb striking the metal fence surrounding the BC facility. MV#1 was then seen / heard revving his engine while still in drive before reversing his vehicle off the curb and attempting to continue on Beacon St westbound. The fence surrounding the BC Facility sustained moderate damage. Tire marks in the grass show the path of MV#1 in direct line to the damaged property. While on scene, I spoke at length with Vanhorne. I did not detect or any alcohol from his breath as he spoke. Vanhorne reported he only consumed one beer tonight at roughly 2030hrs and smoked a little marijuana at 2200hrs. Vanhorne's eyes did not detect any signs of alcohol and he walked with no signs of any impairment. Sgt. Devine administered a horizontal nystagmus test which revealed no signs of impairment. Vanhorne reported he was at a work event and then visited a friend in Brookline. Vanhorne reported his last seizure from epilepsy occurred a little over two years ago. Pictures of the scene were taken and submitted to IT to be attached to this report. MV#1 operator stated he was not injured at this time. MV#1 was towed from the scene by Todys's due to heavy front end damage. Vanhorne was picked up from the scene by Paul Miller his step father. A towed MV form was filled out and placed in the traffic box. Witnesses Cassandra Williams and Bill Peirce observed the crash and stated identical narratives to Ofc. Cadogan's. An Immediate Threat form was filed with the RMV (via Fax and E-mail at approximately 0650hrs 4/24/19)due to his medical history and his explanation with this crash. All streets mentioned are public ways in the City of Newton. It should be noted that the road was wet from rain on this night. Update: on 4/27/19 Sgt.Devine was able to watch the video of the incident from Boston College Police. The video confirmed the statements given by BCPD and witnesses's. Due to the aforementioned statements and the video surveillance of the incident, MV#1 Operator will be cited with Marked Lanes Ch89/4a (MA Uniform Citation #T1441792. Vanhorne will be notified via phone he will be receiving the ticket in the mail.

The force from MV3 crashing into the rear of her vehicle caused MV2's front bumper area to crash into the rear bumper area of MV1. I observed moderate to the front and rear bumper areas of MV2. The vehicle was removed from the roadway by Tody's towing. Thatwisai Partosan reported no injuries and signed a patient refusal with Newton Medics. The operator of MV3, Sofija Garcevic (S74039706) stated she was traveling North on Centre Street towards Cabot Street. Garcevic stated she looked up and observed MV2 in front of her. Garcevic stated she did not have time to stop and crashed into the rear of MV2. I observed moderate to the front bumper area of MV3. The vehicle was removed from the roadway by Tody's towing. Garcevic reported no injuries and signed a patient refusal with Newton Medics. Mv#1 was stopped at the center intersection red light on Grant Ave S/B at Commonwealth Ave. (intersection has two lights, other light prior to carriage rd). Mv#2 was travelling on Grant Ave S/B and rear ended #1. #1 operator stated, after the crash, #2 operator told him the brakes failed. #2 operator told him he would back up, pull over and get out of the way so information could be exchanged. At that time, #2 drove away on Commonwealth Ave W/B. Witness stated he observed the crash while he was at the red light from the opposite direction on Grant Ave N/B. Witness observed #2 rear end #1 and observed #2 drive away. Witness observed and wrote down license plate of #2, MA reg 8SVH20. #1 sustained moderate damage to the driver's side rear that did not require a tow. #1 stated he could feel possible knee and side (oblique) pain. Medics arrived for an evaluation. #1 signed a patient refusal. #2 operator (no injury) was later located on Newtonville Ave (incident report #19017194). #2 sustained heavy front end damage with airbag deployment. Mv#2 was towed from 148 Sumner St by Tody's. #2 issued citation for 90/24 leaving the scene of property damage and 90/10 unlicensed operation.

MV1 was travelling WB on Beacon St when MV2 cut across his lane of travel causing him to strike the passenger side of MV2. MV2 was attempting to turn NB onto Walnut St from Beacon when he was struck by MV1. Operator 2 states he believed he had enough time to make it across the intersection and believes MV1 was speeding. Operator 1 states he was operating at the speed limit (30MPH) and MV2 cut in front of him. Both parties had the green light at the intersection. No injuries reported by any party. Parent of operator 2 signed a patient refusal based upon the operators age. Both vehicles were towed due to air bag deployment. MV1 sustained minor damage to the front end. MV2 sustained moderate damage to the passenger side.

Narrative:
On Sunday, May 5th, 2019 at approximately 1315 hours while working the City of Newton detail for the Walk for Hunger I was standing at my assigned post at the intersection of Centre Street and Willow Street. At the time, the road surface was dry and the weather was cloudy. Traffic was particulary heavy as there were several streets in the area still blocked off for the Walk for Hunger and there were participants still walking the route in the area. My attention was drawn to the intersection of Centre Street and Bowen Street as there was a motor vehicle travelling southbound on Centre Street with their left turn signal on attempting to take a left turn on to Gibbs Street. Gibbs Street was temporarily blocked to non-residential motorists as there were several pedestrians in the road crossing over Gibbs Street. As the motor vehicle attempted to turn, there were several pedestrians in the crosswalk crossing over Gibbs Street. The vehicle had stopped halfway into the northbound lane on Centre Street blocking on coming traffic. The operator appeared to realize Gibbs Street was blocked and attemped to turn the vehicle back into the southbound lane to continue southbound on Centre Street. As the vehicle turned back into the southbound traffic lane, the vehicles rear end collided with Officer Richard Clements, who was operating NPD motorcycle unit 533. Officer Clements was traveling southbound on Centre Street and was passing the vehicles in the southbound traffic lane which were currently all at a standstill. Officer Clements had his emergency lights activated as he was on his way to remove traffic cones from the street to open up all the traffic lanes along the event route. Officer Clements was working the City of Newton detail for the Walk for Hunger assigned to the motorcycle to assist with traffic. Officer Clements front wheel, left side of the motorcycle and left leg collided with the rear end of the motor vehicle. The impact of the crash caused him to fall off the left side of the motorcycle and land on the street face first. At this time I immediately ran from my post to Officer Clements location and radioed dispatch that a motorcycle officer was down and that I needed assistance. As I approached Officer Clements he was face down, unconcious and breathing. I was able to roll Officer Clements into the recovery position and at this time Officer Clements regained conciousness and was able to speak and stated " What happened? What was that lady doing she turned right into me". I advised Officer Clements that he had just been involved in an accident and to stay still and that help was on the way. The operator of the motor vehicle identified as Helen Leibner (S52717988) stopped her vehicle immediatley after the crash and got out of her vehicle to offer her assistance. I advised Helen and several other witnesses and bystanders who were now crowding around Officer Clements and I to back away from the scene and to not move any of the vehicles involved. Officer Selig arrived on scene and was able to help crowd control until further help arrived. Helen and all witnesses were advised to stand by for police to take their statements. While tending to Officer Clements he was disoriented and made several attempts to move his body to which I had to physically hold him still to prevent possible further injuries. While speaking with Officer Clements I noticed blood coming from the inside of his mouth. Officer Clements was in a noticeable amount of pain. He complained of head, back and left leg pain. Officer Clements was wearing a yellow ANSI certified Newton Police reflective jacket and a Newton Police issued motorcycle helmet. Once the paramedics and fire personnel arrived I was able to speak to the operator of the motor vehicle and several other witnesses. Helen stated she was traveling southbound on Centre Street operating her blue 2014 Audi A4 (MA REG 213BC9). Helen states she was attempting to take a left turn onto Gibbs Street and activated her left turn signal. Helen states she had stopped halfway into the northbound lane as several pedestrians were crossing Gibbs Street in the crosswalk at the time. Helen states after the pedestrians cleared she saw the traffic cones blocking the street and realized the street was closed. Helen states she turned her vehicle back into the southbound traffic lane and did not see Officer Clements or the motorcycle until impact. Helen was not injured and her motor vehicle sustained moderate rear end damage. Her motor vehicle was able to drive away from the scene. Witness Adron Gordon (S90357161) stated he was participating in the Walk for Hunger and had just crossed over Gibbs Street when the crash occured. He stated he witnessed the Officer on the motorcycle with the emergency lights on traveling southbound on Centre Street behind the motor vehicle. The motor vehicle stopped in traffic attempting to turn left onto Gibbs Street. He states then the vehicle made a slight turn back into the southbound traffic lane and the motorcycle collided with the rear end of the motor vehicle. He states the Officer then fell off the motorcycle landing in the middle of Centre Street. Witness Elizabeth McCarthy (S00242346) and Katherine Regan (S43486601) stated they both were in the same vehicle together stopped in traffic in the northbound lane of Centre Street just before the set of lights at Centre Street and Bowen Street. Both witnesses stated they do not remember if the driver of the motor vehicle had the turn signal on but they did see her attempt to turn onto Gibbs Street. They stated they also saw the motorcycle with emergency lights activated travel southbound on Centre Street approaching behind the vehicle. Witnesses state the motor vehicle then suddenly turned back into the southbound lane and the motorcycle collided with the passenger side tailight of the motor vehicle. Both witnesses state the motorcycle attemped to turn away from the vehicle to avoid the crash but was unable to do so. Witness Nina Wang (S39460985) stated she was parked on the southbound lane of Centre Street just prior to the crash scene. Nina states she was in her City of Newton DPW marked vehicle with the hazards on as she was working to assist with traffic safety for the Walk for Hunger. Nina states she witnessed the motorcycle travel southbound on Centre Street and pass the vehicles stopped in the traffic lane. Nina states the motorcycle passed the traffic using the left side and had the emergency lights activated. Nina states the motorcycle was just passing through the green light at Centre Street and Bowen Street when the motor vehicle that was stopped at the lights attempting to take a left turn onto Gibbs Street. Nina states the vehicle then abruptly jerked her car to the right and collided with the motorcycles left side. Nina states she heard a loud crash and witnessed the Officer fall off the motorcycle and land face first onto the street. The motorcycle's final resting place was on its left side approximately two feet away from where Officer Clements was located. The motorcycle sustained moderate damage to its left side, including the front wheel well and undercarriage. The motorcycle left scuff marks on the street which were measured and photographed. Several photographs of the scene and damage to both vehicles were taken and submitted to the IT Bureau. The motorcycle was towed by Todys. Centre Street and Bowen Street are both public ways maintained by the City of Newton. The injured motorcyclist, Officer Clements, was transported to Saint Elizabeth Hospital with fire fighters on board. I was able to follow up with Officer Clements on the phone the next day who was admitted over night at the hospital. Officer Clements stated he has no recollection of the accident and can only remember driving down Centre Street to begin to pick up cones to open up all lanes of traffic. Officer Clements stated he suffered a brain bleed, head, back, left foot and right thumb injuries. A Massachusetts Uniform Citation was issued and mailed to Helen for Chapter 89 Section 7A, failure to yield to emergency vehicle, police motorcycle.

Operator of M/V # 1 was operating straight ahead exiting the Boylston Street (EB) off ramp at Ellis Street. The operator states that she never saw the stop sign at the end of the off ramp and proceeded through the intersection when she was struck by M/V # 2 on her driver's side rear door. M/V # 1 sustained moderate damage to the door but the vehicle was still driveable. Operator was issued MA Uniform Citation # T1444908 for Ch. 89/S9 Stop Sign Violation. Below the stop sign there is also a sign that states that oncoming traffic does not stop. Operator of M/V # 2 was operating straight ahead through the above listed intersection attempting to go southbound on to Ellis Street when M/V # 1 suddenly came through the intersection. The operator could not avoid M/V # 1 striking it on the driver's side rear door. The vehicle sustained moderate damage to the right side of the front bumper area but it was still driveable. The operator had her dog, Coconut, in the rear of the vehicle at the time but the dog appeared to be fine. No injuries reported by either operators.

The operator of MV#1 stated he was travelling westbound on Watertown St when MV#2 rolled backwards from its parking spot and struck him. MV#1 sustained moderate damages to its front right side bumper and its rear right side doors. There were no reported injuries to the operator of MV#1. The tow truck operator ("Todisco Towing" truck with a ramp bed MA Repair plates 98BD) stated he was loading up MV#3 (An unregistered red colored BMW sedan being retrieved for auction bearing VIN WBABS33403PG88984) onto his inclined ramp bed when the tow hook on MV#3 broke causing MV#3 to roll backwards striking MV#2 which in turn pushed MV#2 into MV#1. Both MV#3 and MV#2 were unoccupied at the time of this accident and both vehicles were being towed for auction. There were no visible damages to the rear end of MV#3. There were minor damages to the rear left side of MV#2.

On Wednesday, May 29, 2019 while assigned to Traffic unit N525, I responded to the area of Centre Street and Washington Street, Newton for a report of a motor vehicle crash involving a school bus. The weather at the time of the crash was overcast and the road surface was dry. Centre Street and Washington Street are both public ways maintained by the City of Newton. The operator of MV1, Norma Cedeno (S60008138), stated she was operating a 2016 Dodge Caravan (MA SPN: 104893) around the Washington Street/Centre Street rotary. Cedeno stated as she was traveling in the left turn lane on Centre Street, she attempted to travel straight (N) through the intersection at Washington Street towards 255 Washington Street. Cedeno stated the front passenger side of her vehicle crashed into the driver side of MV2. MV2 was attempted to continue left around the rotary and was located on the passenger side of her vehicle. Cedeno reported no injuries. I observed minor damage to the front passenger side tire area of MV1. Cendeno's passenger, Cristina Velez (S04276767), reported not injuries. The operator of MV2, Mary Sanker (S62890912), stated she was operating her 2016 BMW 328XI around the Washington Street/Centre Street rotary. Sanker stated as she was continuing through the rotary in her lane, the vehicle traveling in the lane to her left (MV2) entered her travel lane in an attempt to travel straight towards 255 Washington Street and crashed into the driver side of her vehicle. I observed moderate damage to the front and rear driver side doors of MV1. Sanker reported no injuries. The operator of MV1 was cited for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care)

The operator of vehicle 1 stated he was traveling North on Parker Street when he began to brake and slow down due to traffic stopped ahead of him. His brakes started pulsing and he lost control of the vehicle and could not stop. This caused him to crash into vehicle 2 which was stopped in traffic and then caused vehicle 2 to crash into the rear of vehicle 3. This caused moderate damage to vehicle 1 which had to be towed. It caused major damage to vehicle 2 however due to the location of the damage it was able to be driven away, and minor damage to the rear of vehicle 3 which was also able to be driven away. The operator of vehicle 2 stated she was sitting in traffic on Parker Street when vehicle 1 rear ended her, causing her to then rear end vehicle 3. The operator of vehicle 3 stated he was sitting in traffic on Parker Street when vehicle 2 rear ended him. The operator of vehicle 3 stated he had minor neck pain and was evaluated by Cataldo medics and signed a patient refusal.

On Thursday, June 6, 2019 while assigned to Traffic unit N525, I responded to the intersection of Waverly Avenue and Franklin Street for a report of an accident involving a City of Newton school bus. The weather at the time of the accident was clear and sunny. The road surface was dry. Waverly Avenue and Franklin Street are both public ways maintained by the City of Newton. The operator of MV1, Dorothy Wensink (S40227061), stated she was operating her 2010 Toyota Prius (MA: 538EV6) Eastbound on Franklin Street towards Waverly Avenue. Wensink stated she came to a stop at the stop sign on Franklin Street (E) at Waverly Avenue. Wensink stated as she began to travel through the intersection, a school bus traveling on Waverly Avenue (S) crashed into the rear driver side door area of her vehicle. Wensink reported no injuries on scene. I observed moderate damage to the rear driver side area of MV1. Wensink was able to driver her vehicle away from the scene. MV2 was a City of Newton contracted yellow school bus (MA BUN: 39173) transporting students attending the Ward Elementary School. The bus was returning to the Ward School from a field trip. The operator of MV2, Antonio Goncalves (S54275856), stated he was operating the school bus Southbound on Waverly Avenue towards Franklin Street. Goncalves stated MV1 abruptly accelerated across the intersection from Franklin Street. Goncalves stated he was unable to avoid making contact with MV1 and crashed into the rear driver side of the vehicle. I observed minor damage to the front middle and passenger side bumper area of MV2. The following Ward Elementary School student, teachers, and parents were traveling on the school bus at the time of the accident and reported no injuries. Students: Avishai Arbili, Eleanor Chen, Maeve Donovan, Esat Efendigil, Mark Elentuck, Julian Ismay, Rhea Khanna, Griffin Kirby, Ori Layani, Priscilla Le-Ngo, Callum McLaughlin, Carolina Muniz, Emily Ney, Hrithik Ponduru, Levi Rosenberg Van Gameren, Zachary Schwarzberg, Ava Smoller, Matthew Somers, Lillian Welford, William Balerna, Alexandra Bogdanova, Randi Green, Christopher Iatrou Johnson, Charlotte Nichols, Henry Pozen, Sofia Shadah Alcalde, and Zoey Steinberg. Teachers: Katherine Goldman and Lauren Ludman. Parents: Enrique Shadah, Brian Rosenberg and Xinxin Wang. The Principal of the Ward Elementary School, Becca Brogadir, responded to the crash scene and signed a patient refusal for all of the students with Newton Medics. The students, teachers, and parents, then walked back to the Ward Elementary School with Principal Brogadir. The school bus was able to drive away from the scene. Once the students arrived at the Ward Elementary School, the nursing staff there evaluated each child before they could return to class. The school staff stated they would make notifications to the parents of the children involved in the accident. After speaking with all parties involved, the operator of MV1 was cited in hand for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Pictures were taken of both vehicles and submitted to the IT Bureau.

Operator of MV1 was driving northbound on Cypress Street and was trying to apply his brakes to slow down in traffic. His brakes failed and he turned left onto Paul Street, striking a curb in front of #17 to stop his vehicle. He was evaluated by Cataldo and signed a patient refusal. The operator of MV1 stated she was traveling Westbound in the right lane on Washington St. approaching the red light at the intersection of Lowell Ave. She stated MV2 was traveling Westbound on Washington St. in the left lane, and then came into her lane striking the left side of her vehicle. The operator of MV1 stated she told the operator of MV2 out of the window that she just side swiped her, and she responded that she did not. The operator of MV1 stated the light then turned green, at which time MV2 took a left onto Lowell Ave Southbound and never pulled over. I viewed the damage to MV1 which consisted of large moderate scratches down the length of both doors on the left side of the vehicle, enough for one to realize there had been a collision. Witness Jeff Bennett flagged down the operator of MV1 and provided the registration of MV2 as MA Reg 531PX6, a Black Mercedes SUV. The registered owner of MV2 matches the description provided as a blonde female with a pony tail, possibly in her 30's. I left a message with the operator of MV2, Colleen Butaney, who returned my call and stated she was traveling Westbound on Washington St. in the left lane approaching the red light at Lowell Ave. She stated she started to proceed into the right lane but saw MV1 and turned back into the left lane. She stated the operator of MV1 rolled down her window and stated that she just side swiped her car, to which she responded "no I didn't but I am happy to pull over". She stated at this time the light turned green and MV1 continued straight on Washington St. and didn't pull over. I ALSO SPOKE TO THE OPERATOR OF MV 2 IDENTIFIED AS JANE HOPKINS-WALSH WHO STATED THAT SHE DID BACK OUT OF HER DRIVEWAY AT THE TIME FRAME IN QUESTION AND STATED SHE MAY HAVE STRUCK MV 1 BUT DID NOT REALIZE SHE HAD DONE IT. Operator of MV1 stated he was traveling Northbound on Walnut Street when MV2 was traveling Eastbound on Linwood Ave, crossing Walnut St. As MV1 approached the intersection MV2 was crossing and MV1 struck MV2. On Monday, July 1, 2019 while assigned to Traffic unit N525, I responded to the area of 802 Walnut Street, Newton for a report of a motor vehicle accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Walnut Street is a public way maintained by the City of Newton. The operator of MV1, Dayana Martinez (S96710554), stated she was operating her 2019 Toyota Rav 4 (MA: 934RJ9), Northbound in the area of 802 Walnut Street, Newton. Martinez stated as she was coming around the bend the sensor system for her vehicle activated and she attempted to pull her vehicle away from the double yellow line. Martinez stated the vehicle locked up and she made contact with MV2. Martinez reported no injuries on scene. I observed moderate damage to the front driver side area of MV1. The operator of MV2, Gerard Kenneally (S75917793), stated he operating his 1999 Ford E150 (MA: 3AL821), Southbound in the area of 802 Walnut Street, Newton. Kenneally stated MV2 was traveling Northbound on Walnut Street and crossed over the double yellow line and side swiped the driver side of his vehicle. Kenneally reported no injuries on scene. I observed moderate damage to the driver side of MV2 from the driver door to the rear bumper area. I spoke with a witness to the accident, Bryan Stewart, via telephone. Stewart stated he was traveling Southbound on Walnut Street behind MV2 at the time of the accident. Stewart stated he observed MV1 drive over the double yellow line and into their travel lane. Stewart stated 3/4 of the vehicle was in the Southbound lane while it was traveling Northbound. Stewart stated MV1 crashed into MV2. Stewart stated if MV2 was not in front of him then MV1 would have crashed into him. After speaking with all parties involved, Martinez was mailed Massachusetts Uniform Citation T1272998 for Chapter 89, Section 4A (Marked Lanes).

On 07/16/19 at approximately 09:38 I responded to a two vehicle crash on Needham St. outside of #141. The Operator of Veh #1 stated she was turning left on Needham St when the South bound traffic stopped to left her turn. As she turned, Veh#2 drove South down the multi-turn lane and they collided nearly head on. Heavy damage and air bag deployment to Veh #1. The operator was checked out by Medics and signed a patient refusal. The Operator of Veh #2 stated she was driving down the multi-turn lane South on Needham St.. She was preparing to take a left onto Jaconnet St when Veh #1 pulled out of parking lot and collided with her vehicle. I observed moderate to heavy damage to Veh #2 and noted that the vehicle didn't have its directional on. The operator of Veh #2 complained to left leg pain and was transported to Newton-Wellesley Hospital By Ambulance #1. Both vehicles were towed by Tody's Towing. At the conclusion of my investigation it appears as if both vehicles were at fault during this crash; Veh#1 for not yielding to right of way and Veh #2 for traveling too soon in a turn lane, no directional and driving too fast for the heavy traffic conditions. Both operators were given verbal warnings and there will be no citations issued at this time.

On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

On 07/24/19 at 08:06 hours, I responded to a two car motor vehicle accident at Dedham and Parker Streets with no reported injuries. Operator of MV1 was stopped at the stop sign on Parker Street (SB) at Dedham Street waiting to take a left turn on to Dedham Street. That is when he was struck in his rear end by MV2. MV1 sustained moderate damage to the rear bumper and hatch area but it was still driveable. Operator of MV2 states she was behind MV1 on Parker Street but did not see that vehicle in front of her. She states she was also attempting to take a left turn on to Dedham Street. She then started to accelerate and struck MV1 in the rear end. I observed moderate damage to the entire front bumper and grille but mainly to the driver's side. The owner decided to have the vehicle privately towed after on Howe Road. Operator was issued MA Uniform Citation # T1445344 for Newton City Ordinance 19-75 Failure to use care in starting and stopping.

On 07/25/19 at 10:28 hours, I responded to Centre Street and Boylston St off ramp (WB) for a two car motor vehicle accident with no injuries reported. Operator of MV1 states he was traveling straight ahead on Centre Street (NB) when he observed MV2 inching out of the Boylston Street off ramp. He was not able to stop in time and the two vehicles struck one another. I observed heavy damage to the passenger front bumper and tire area and the vehicle was towed by Tody's. Operator of MV2 states he was exiting the Boylston Street off ramp (WB) and was going to attempt a left turn on to Centre Street after stopping for the stop sign. He then slowly pulled out into the intersection due to National Grid trucks blocking his view and that is when he came in contact with MV1 on his driver's side front bumper. I observed moderate damage to the entire front bumper but the vehicle was still driveable. I did observe National Grid trucks parked at the intersection setting up to do construction work. One truck was parked on the left side of the off ramp and the other was parked on the side where the crash occurred. This did obstruct the operator of MV2's view when pulling out.

On 07/28/19 at approximately 12:26 I responded to a two vehicle crash at the intersection of Needham St and Tower Rd. The operator of veh #1 stated she was driving Southbound on Needham St. when veh #2 pulled out of Tower Rd. from her right and collided with her car. The side airbags went off in veh #1 but the driver stated she wasn't hurt. She was checked out and signed a patient refusal with the Medics. Moderate to heavy damage to veh #1 as it was towed by Tody's towing. The operator of veh#1 has a License and Registration issued by the Department of The State (Brazilian Consulate) and isn't in our in house system to plug into this report. The operator of veh #2 stated she was pulling out of Tower Rd attempting to go left (Northbound) on Needham St. and didn't see veh #1 passing as she turned. On 07/29/19 at 11:13 hours, I responded to the Dunkin Donuts parking lot located at 138 Needham Street for a three car motor vehicle accident. Operator of MV1 stated at first she could not recall what occurred during the accident due to lightheadedness at the time. After, she then stated after pulling in to the Dunkin Donuts lot she was attempting to back in to a parking spot in the back of the lot. While backing up, she attempted to hit the brake pedal but must have pushed the gas pedal instead. The vehicle then struck parked/unoccupied MV2 and MV2 was then pushed in to parked/unoccupied MV3. The operator was checked out by Cataldo Ambulance and signed a patient refusal. MV1 sustained moderate driver's side rear bumper area damage but it was still driveable. MV2 sustained damage along both sides of the vehicle and a rear broken axle and it was towed by Tody's. MV3 sustained scrapes and dents to the entire driver's side but the vehicle was driveable.

Operator Motor Vehicle # 2 stated that she attempted to take a left turn into a driveway so she could turn around and thought she could make a U Turn instead. Operator of Motor Vehicle #2 stated that she took the left and did not see the vehicle that was in the left lane next to her and struck vehicle # 2. Motor Vehicle #2 sustained moderate drivers side, side damage, and motor vhehicle #1 sustained moderate front end damage. Both parties reported no injuries and signed medical refusal forms.

Motor Vehicle 1 (MV1) was traveling northbound on Centre St. at the intersection of Walnut St. when motor vehicle 2 (MV2), which was also traveling northbound on Centre St., attempted to go straight from the left turn only lane. MV2 then sideswiped MV1, running MV1 off the road and into the curb. The operator of MV1, Drew Brookins, stated that the operator of MV2 pulled over at first, but when traffic cleared, immediately got back into the car and left at a high rate of speed heading northbound on Centre St. Mr. Brookins attempted to follow MV2, and observed MV2 take a right onto Paul Street (eastbound), but was unable to continue to follow MV2, as MV1's front drivers side wheel was blown out as a result of colliding with the curb. MV1 sustained drivers side damage, as well as a blown out right drivers side tire. MV2 likely sustained moderated drivers side damage, and possibly damage to the rear bumper. Mr. Brookins was able to get the license plate of MV2 (MA Reg. 9LE764), which comes back to a female (Desiree Allman) out of Braintree. Mr. Brookins described the operator of MV2 as a white male with a thin build, dirty blond hair and scruffy beard. Dispatch contacted Braintree PD who was able to provide a telephone number that is believed to be Ms. Allman's. I attempted to contact Ms. Allman, however, the number went directly to voicemail. I left a message asking Ms. Allman to call the NPD so that we could find out who was operating her vehicle at the time of the crash. Traffic Bureau Update (Officer Gaudet): On August 6, 2019, I attempted to make contact with the registered owner of MV2, Desiree Allman (S85529796), with a negative result. A Hit and Run inquiry was mailed to the Allman. As of August 8, 2019, I have left several voice mails for Ms. Allman, she has not returned my calls. Ms. Allman had a hearing today in Hingham District Court. I contacted them to confirm we have her current contact information.

Narrative:
On Thursday, August 8, 2019, while assigned to Traffic unit N525, I responded to the area of Centre Street and Mt. Ida Street for a report of a 3 car motor vehicle crash with entrapment. The weather at the time of the crash was clear and sunny. The road surface was dry. Centre Street and Mt. Ida Street are both public ways maintained by the City of Newton. On arrival, I was updated by Sgt. Chisholm that one of the vehicles (MV3) had a female operator that was entrapped as a result of the collision. She was freed from the vehicle by the Newton Fire Department and transported to Beth Israel Hospital. Another operator (MV2) was transported to Newton Wellesley Hospital with minor injuries. A third operator (MV1) remained on scene with no injuries. With the assistance of Officer Wade we conducted a survey of the crash scene. We observed a large debris field at the intersection of Centre Street and Mt. Ida Street with debris from all three vehicles. The point of impact was observed to be on Centre Street (N) before Mt. Ida Street. There were fresh scrapes in the pavement as a result of the colission. We also observed skid marks from MV1 measuring 57 feet 8 inches from the start of it's skid to the vehicles final rest MV1 crashed into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane of Centre Street and crash into MV3 as it was passing by. The impact from MV2's front end crashing into MV3's driver side resulted in the operator of MV3 becoming entrapped. MV2 and MV3 came to final rest on Wesley Street (W) at Centre Street (S). The driver door of MV3 was inaccessible. The collsion resulted in air bag deployment for MV2 and MV3. I observed moderate damage to MV1's front grill/bumper area. I observed substantial damage to the front and rear of MV2 and substantial damage to the front and driver side of MV3. Tody's responded and removed MV2 and MV3 from the roadway. Photos were taken of the scene and submitted to the IT Bureau. Officer Martin of the Watertown Police Department was requested by Sgt. Lee to respond to the scene and assist with a commercial truck inspection of MV1. Officer Martin contacted Trooper Grey of the Massachusetts State Police Truck Team who also responded to assist with the truck inspection. Sgt. Lee remained with Officer Martin and Trooper Grey during the inspection. MV1 was relocated to Richardson Street for the inspection. Trooper Grey determined the truck was in compliance with all required paperwork and it passed it's mechanical inspection. MV1 was removed from the roadway by a state tow. I spoke with the operator of MV1, William Tisbert (S24792655). Tisbert stated he was operating a 2006 KW W900 Semi Truck (NH SEMI: AP2638) Northbound on Centre Street. Tisbert stated as he was approaching Mt. Ida Street, a vehicle came out of no where and he hit it. Tisbert stated there was no one in front of him and then a vehicle came off of a side street (Mt. Ida Street) and attempted to turn left in front of him to return to the side street. Tisbert stated he could not avoid hitting the vehicle. Tisbert crashed into the rear of MV2. MV2 then crossed into the opposite lane and crashed into another vehicle. Tisbert remained on scene and reported no injuries. Tisbert stated he was wearing his seatbelt at the time of the accident and was not distracted. Tisbert later admitted to crashing into the rear of MV2. I spoke with the operator of MV2, Janet Vanbuskirk (S59829146), at Newton Wellesley Hospital. Vanbuskirk stated while operating her 2004 Toyota Highlander (MA: RS 529Y), she was stopped on Centre Street (N) at Mt. Ida Street. Vanbuskirk had her left directional on and was waiting for traffic in the Southbound lane to clear to take a left turn onto Mt. Ida Street. Vanbuskirk stated MV1 crashed into the rear of her vehicle. As a result of the impact, Vanbuskirk's vehicle traveled into the opposite lane and crashed into the driver side of MV3. Vanbuskirk reported minor injuries to her neck, left arm, and right hand. I spoke with the operator of MV3, Madeleine Karolian (S23979811), at Beth Israel Deaconess Medical Center in Boston. Karolian stated she was operating her 2016 Toyota Prius (MA: EV5746) Southbound on Centre Street. Karolian stated MV1 crashed into MV2, and MV2 crashed into her vehicle. Karolian stated she did not have time to avoid the collision. Karolian stated she had minor injuries to her neck and chest. Karolian's vehicle had a dash cam recording at the time of the accident. I viewed the footage with her through an app on a cell phone. The footage shows MV3 traveling Southbound on Centre Street on August 8, 2019 at 1308 hours. Ahead of MV3 in the Northbound lane, MV2 is seen stopped on Centre Street before Mt. Ida Street with it's left directional activated. I observed MV1 travel Northbound on Centre Street and crash into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane and crashed into the driver side of MV3. The footage then ends. A copy of the footage was emailed to my work email. The footage was then forwarded to IT Director Steve Smith to be added to the media for this report. As a result of my investigation, the operator of MV1, William Tisbert, will be cited for Newton City Ordinance Chapter 19, Section 71 (Following too closely). Massachusetts Uniform Citation T1270942 was completed and mailed to Tisbert's residence.

The operator of MV#1 stated he was stationary on Centre St northbound when he was struck from behind by MV#2. MV#1 sustained moderate damages to its rear bumper. There were no reported injuries to the operator of MV#1. The operator of MV#2 stated he was stationary on Centre St northbound when he was struck from behind by MV#3 which in turn pushed him into MV#1. MV#2 sustained moderate damages to its rear and front end. The operator of MV#2 reported non-incapacitating injuries to his back and right shoulder area. He was evaluated by Newton Paramedics and refused medical attention. The operator of MV#3 stated he was travelling northbound on Centre St when he looked to his right at a dump truck and took his vision off the street for a second when he struck MV#2 which in turn pushed MV#2 into MV#1. MV#3 sustained heavy front end damages and had both front airbags deployed. The operator of MV#3 stated both of his thumbs were hurting from the airbag deployment. He was evaluated by Newton Paramedics and refused medical attention. The operator of MV#2 does not have a driver's license and only provided a Brazilian passport. He was issued in hand MA uniform citation #T1268350 for violation of c90 s10 unlicensed operation. A licensed operator (MA license #S48386590) took possession of MV#2 and transported the operator of MV#2 back to his home. Based on the statements made to me I issued in hand to the operator of MV#3 MA uniform citation #T1268351 for violation of C.O 19-71 following too closely. MV#3 was towed by AAA towing and the operator of MV#3 was transported home by his parents.
