MV#1 Stated while traveling Westbound on Watertown St at the intersection of Eliot Ave his vehicle was struck on the driver side by MV#2. MV#1 stated MV#2 was attempting to cross Watertown St from Eddy St when MV#2 entered his lane of travel.MV#1 had heavy front and driver side damage following the collision. MV#2 Stated while traveling Northbound on Eddy St she attempted to cross Watertown St and on to Eliot Ave. MV#2 stated MV#1 had dim headlights and appeared to be traveling at a high rate of speed. MV#2 stated the front passenger side of her vehicle struck MV#1 on the driver side. Both MV#1 and MV#2 were towed from the scene due to heavy damage to both vehicles. All parties signed a medical refusal on scene and stated the were not injured at this time.

On 2/25/2019 at approx 1130hrs while assigned to 497 I responded to the intersection of Commonwealth Ave and Chestnut St for a two car crash. Upon arrival I met with the operator of Ma reg RDTAIL ,Steve MCCULLOUGH who stated he was traveling NB on Chestnut St through the green light when he was suddenly struck by the operator of Ma Reg 298CBV George DIARBAKERLY who was travelling EB on Commonwealth Ave. I spoke with DIABAKERLY who initially stated he thought he had the green light but perhaps it changed, than stated maybe he was too far back when he saw the green light. MCCULLOUGH gave me the names of two people who had stopped , Matt DOLITKA 617-719-7621 and Dr J LAUFFLER 617-968-6343. I was able to speak with DOLITKA who stated he was travelling NB on Chestnut ST one car back from MCCULLOUGH, they were traveling through the green light which was solid green, had not just changed, when he saw MCCULLOUGH get hit by DIARBAKERLY at a high rate of speed, DIARBAKERLY struck MCCULLOUGH than bounced off and went up onto the island. DIARBAKERLY issued Mass Uniform Citation T1441861 for violation MGL 89/9 Failure to Stop, Red Light. All parties signed patient refusals with Cataldo. both vehicles towed by Todys.

Operator of MV#1 stated she was travelling east in the area of 1507 Washington St in the southbound lane. She began to slow down in traffic when she was rear ended by MV#2. MV#1 suffered minor damage to the back and was able to be driven from the scene. Operator of MV#2 stated she was also in the southbound lane and traffic was travelling at a normal speed when she turned to look left to switch lanes. She stated at that moment "traffic began to slow down into gridlock" and she could not stop in time. She rear ended the back of MV#1. MV#2 suffered major damage to the front end and was towed from the scene by AAA (CJ's Auto.) No injuries.

Mrs. Weiss stated that she was attempting to back out of her driveway at 285 Crafts Street when her vehicle accelerated for no reason "like a bat out of hell." The vehicle then crossed Crafts street at a high rate of speed, jumped the curb, crossed the large grass island, and only stopped when the vehicle crashed into a tree. Mrs. Weiss was uninjured and signed a patient refusal. The vehicle sustained moderate damage to the tires, undercarriage, and rear bumper and was towed from the scene by Todys. Photos were taken of the damaged city tree and were placed in the IT bureaus mailbox to be attached to this report. Based on the facts that Mrs. Weiss could not explain the reason for the cars acceleration, the distance the vehicle traveled without stopping, and the close proximity of the crash to an elementary and middle school I filed a request for Immediate Threat License Suspension/Revocation. All proper paperwork as well as a copy of this report were faxed to the Registry of Motor Vehicles.

MV1 was travelling WB on Beacon St when MV2 cut across his lane of travel causing him to strike the passenger side of MV2. MV2 was attempting to turn NB onto Walnut St from Beacon when he was struck by MV1. Operator 2 states he believed he had enough time to make it across the intersection and believes MV1 was speeding. Operator 1 states he was operating at the speed limit (30MPH) and MV2 cut in front of him. Both parties had the green light at the intersection. No injuries reported by any party. Parent of operator 2 signed a patient refusal based upon the operators age. Both vehicles were towed due to air bag deployment. MV1 sustained minor damage to the front end. MV2 sustained moderate damage to the passenger side.

On 05/15/2019, while assigned to N494, I, Officer Conary, responded to a MVA at 109 Adam Street, a public way in the City of Newton. Upon arrival, I observed a utility pole broken in half, wires hanging in the street, and also a transformer on the ground. Fire and medics responded. Other police units were able to block the street from traffic. I spoke to witness, HARRIS, who stated that she was traveling Southbound on Adam Street when she saw MV1 in the wrong lane traveling toward her at a high rate of speed. HARRIS stated that MV1 then turned into the utility pole located outside of 109. MV1 took out the pole, side swiped MV2 and then hit MV3 head on. Operator of MV1 stated that he was traveling Northbound on Adam Street when he sneezed. At this point, he said his eyes were off the road and the next thing he could remember was being in the parking lot of 109 Adam Street after the accident. There was significant damage to the front of MV1 with airbag deployment. Operator of MV1 was checked out by the medics for a head injury and signed a refusal. Operator of MV1 was asked if he had consumed any alcohol or taken any medication today and he declined. Officer Spinney (N491) asked if he was willing to do a Field Sobriety Test, and he stated yes. Operator of MV1 did not show any signs of intoxication. Operator of MV2 stated that he had parked his car outside of 109 Adam Street and was sitting in the driver seat when he was hit by MV1 and also the utility pole. Operator of MV2 was able to exit his car. He was evaluated by the medics and signed a refusal. MV2 has significant damage to the right side and windshield. MV3 was parked in the lot of 109 Adam Street facing Southbound. MV1 hit MV3 head on. There was significant damage to MV3 front end. The electric meter attached to 109 Adam Street was ripped off the house. There are no signs of damage to the house to report at this time. MV1 was towed by Tody's. Eversource arrived on scene. Operator of MV1 was issued a citation for marked lanes violation. Pictures were taken of the scene and will be submitted to IT.

Operator of MV1 states she was traveling EB on Comm Ave. She states that while attempting to travel straight across the intersection MV2 cut right in front her striking the front passenger side of MV1. Motor Vehicle 1 (MV1) was traveling northbound on Centre St. at the intersection of Walnut St. when motor vehicle 2 (MV2), which was also traveling northbound on Centre St., attempted to go straight from the left turn only lane. MV2 then sideswiped MV1, running MV1 off the road and into the curb. The operator of MV1, Drew Brookins, stated that the operator of MV2 pulled over at first, but when traffic cleared, immediately got back into the car and left at a high rate of speed heading northbound on Centre St. Mr. Brookins attempted to follow MV2, and observed MV2 take a right onto Paul Street (eastbound), but was unable to continue to follow MV2, as MV1's front drivers side wheel was blown out as a result of colliding with the curb. MV1 sustained drivers side damage, as well as a blown out right drivers side tire. MV2 likely sustained moderated drivers side damage, and possibly damage to the rear bumper. Mr. Brookins was able to get the license plate of MV2 (MA Reg. 9LE764), which comes back to a female (Desiree Allman) out of Braintree. Mr. Brookins described the operator of MV2 as a white male with a thin build, dirty blond hair and scruffy beard. Dispatch contacted Braintree PD who was able to provide a telephone number that is believed to be Ms. Allman's. I attempted to contact Ms. Allman, however, the number went directly to voicemail. I left a message asking Ms. Allman to call the NPD so that we could find out who was operating her vehicle at the time of the crash. Traffic Bureau Update (Officer Gaudet): On August 6, 2019, I attempted to make contact with the registered owner of MV2, Desiree Allman (S85529796), with a negative result. A Hit and Run inquiry was mailed to the Allman. As of August 8, 2019, I have left several voice mails for Ms. Allman, she has not returned my calls. Ms. Allman had a hearing today in Hingham District Court. I contacted them to confirm we have her current contact information.

On September 16, 2019 at approximately 17:50 hours I, Officer Guarino, responded to the area of 749 Beacon St. for a report of a motor vehicle crash involving a bicycle. Upon arrival with other units, we determined that MV 1 was parked in front of 749 Beacon St.The operator of MV1 opened the door to her vehicle to exit and her door struck a passing bicyclist. Traffic on Beacon St. westbound was stopped, the bicyclist fell off of the bicycle and sustained injuries and was transported to Newton-Wellesley Hospital for further evaluation.As the bicyclist was falling, he struck the rear of MV2, which did not appear to have any damage. The operator of MV1 said she did not see the bicyclist, as she went to exit her vehicle. Witness 1, who was stopped in traffic said that the bicyclist was traveling at a high speed and understood how MV1 did not see the bicycle coming. Officer D. Siegal transported the bicycle to the Newton Police Department and secured it in support services.

THE OPERATOR WAS UNINJURED AND THE VEHICLE HAD LITTLE TO NO DAMAGE TO THE FRONT RIGHT PASSENGERS SIDE. THE PEDESTRIAN, MICHAEL MCKEAN, STATED HE RAN OUT TO CROSS THE STREET AND DIDN'T SEE MV1. HE SUFFERED A MINOR RIGHT FOOT INJURY AND WAS TRANSPORTED TO ST. ELIZABETHS. BY MEDIC 1. ALL PASSENGERS OF MV1 REPORTED THAT THE OPERATOR WAS NOT SPEEDING AND THE PEDESTRIAN DARTED INTO THE STREET. THE OPERATOR HAD DASH CAM FOOTAGE OF THE ACCIDENT, AND IT SHOWED MV1 TRAVELING WESTBOUND ON BEACON ST APPROACHING THE VEHICLE PULLED TO THE SIDE OF THE ROAD. IT THEN SHOWED THE PEDESTRIAN RUN INTO THE STREET WITHOUT LOOKING, GETTING STRUCK BY THE FRONT PASSENGER SIDE OF MV1 AND FALLING TO THE GROUND. THE OPERATOR WILL BE E-MAILING THE FOOTAGE TO BE ATTACHED TO THIS REPORT. 1 PHOTO WAS TAKEN OF THE VEHICLE AND SUBMITTED TO THE IT BUREAU.

Narrative:
The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave making a left turn onto Morton St when he was struck by MV#2. The operator of MV#1 stated he was unsure of where MV#2 came from. There was a strong solar glare facing westbound on Commonwealth Ave during this accident. MV#1 sustained heavy driver's side front end damages. Both front airbags were deployed. The operator of MV#1 stated he was not injured. The front passenger of MV#1 was not injured and had already left the scene to attend school. The driver's side rear passenger sustained a bleeding nose from this accident. She was unsure what caused her injuries because it happened too quickly. She was evluated by Newton Paramedics and signed a patient refusal of treatment. The initial point of impact for both vehicles was in the middle intersection of Commonwealth Ave at Morton St. There were broken vehicle parts and tire skid marks starting from the point of impact and onto the berm on Commonwealth Ave on the carriage lane side. MV#2's final resting spot was directly accross from 808 Commonwealth Ave. MV#2 sustained heavy front end damages to its driver's side wheel (Broken axle) and flat front right side tire. MV#2's driver's side airbag was deployed. The operator of MV#2 was transported by Newton Paramedics to the Boston Medical Center. I spoke to the operator of MV#2 via telephone and she stated she was travelling westbound on Commonwealth Ave when she was struck by MV#1 and caused her swerve onto the carriage lane berm. The operator of MV#1 is 6.5 months into her pregnancy and sustained neck, back, bruises and left hand injuries. She was transported to Boston Medical Center and was later discharged the same day. She was provided with this accident report number and was advised to contact her insurance provider. I spoke to the witness who was at the intersection of Cedar St and Commonwealth Ave. The witness stated MV#1 was travelling eastbound on Commonwealth Ave and had stopped, attempting to make a left turn onto Morton St. The witness stated she then observed MV#2 travelling westbound on Commonwealth Ave and struck MV#1. The witness stated she believed MV#2 was travelling at a high rate of speed because it continued to travel onto the berm after striking MV#1. I took digital photographs of the damaged berm on Commonwealth Ave. Based on the statements made to me, I issued in hand to the operator of MV#1 citation #T1268357 for violation of c89 s8 failure to yield when turning left. Both vehicles were towed by "Tody's Towing".

VEHICLE #1 HAD MINOR LEFT REAR DAMAGE AND VEHICLE WAS OPERATIONAL. VEHICLE #2 HAD EXTENSIVE FRONT END DAMAGE AND WAS TOWED BY TODYS. NEWTON FIRE RESPONDED AND PLACED SPEEDY DRY ON VEHICLE FLUIDS THAT LEAKED ONTO STREET. ALL PARTIES REPORTED NO INJURIES. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES.

The operator of vehicle 1 stated that she was traveling North on Winchester Street when vehicle 2 came out from her right side at a high rate of speed and cut her path of travel off. It happened too quickly for her to stop and she crashed head on into the driver side front corner of vehicle 2. She stated she was on the left side of the lane but was traveling with the normal flow of traffic. This caused minor damage to vehicle 2 which was able to be driven away, vehicle 1 sustained major damage to the front bumper and hood and needed to be towed. The operator of vehicle 2 stated he was turning left onto Winchester Street after he had stopped at the stop sign. He stated traffic had stopped to let him turn, he proceeded to do so and vehicle 1 came from the far side of the lane, going around stopped traffic, and crashed into his car.

Vehicle #1 Was Traveling Southbound on Parker St. Vehicle #1 collided with a pedestrian on it's left drive side in a marked crosswalk in front of 21 Parker St. The operator of vehicle #1 stated she saw the pedestrian through her driver's side window at the same time she heard a loud bang. The operator of vehicle #1 stated she noticed her mirror was gone and realized she had hit something so she stopped. The pedestrian in the crosswalk was crossing from his residence at 21 Parker St. on the west side of Parker to the east side of Parker St. in a marked crosswalk. The pedestrian was approximately 2 feet from the middle divider when he was hit by the mirror of vehicle #1. It should be noted that the pedestrian was wearing all dark clothing. The Pedestrian appeared to suffer an injury to his arm and his body and was transported to Beth Israel by Cataldo. The Witness stated he did not see the crash, but just prior to it he saw an unknown vehicle driving at a high rate of speed northbound on Parker St. and the vehicle may have beeped at the pedestrian in the crosswalk causing him to step back into the southbound travel lane where the crash happened. Photographs were taken of the scene and submitted to the IT bureau to be attached to this report. The operator of Vehicle #1 was cited with MGL Ch. 89-11 fail to yield to pedestrian in a crosswalk (Citation #T2079123).

The operator of vehicle 1 stated she was traveling West on the Washington Street bridge over the Mass Pike when vehicle 2, a gray SUV attempted to change lanes, as this happened the front passenger side quarter panel of vehicle 2 came in contact with the rear drivers side quarter panel of vehicle 1. This caused minor scratches to vehicle 1. The operator of vehicle 1 attempted to pull over and exchange information with the other driver. The operator of vehicle 2 continued driving away and got onto the Mass Pike, during this process the operator of vehicle 1 and the passenger of vehicle 1 were able to get the license plate number of vehicle 2 which was NY REG:T797800C. In an attempt to locate and identify the driver of vehicle 2 I ran NY REG T797800C which comes back registered to a short term rental company called Flexdrive. I called a phone number on their website 8663539399 which is listed as the number to call if you are in an accident with one of their vehicles. I was only able to speak to a service technician, a Tom Louis of ARI Fleet Service 18002272273, who told me the vehicle was out of the Flexdrive North Dighton Mass hub located at 123 Williams Street North Dighton MA 02764.Tom also mentioned the location would most likely not be open again until Monday. He did not have any other information regarding who may be operating the vehicle. I have attempted to find contact information on this North Dighton location which I have had negative results so far. I have also emailed the Flexdrive customer service department. To be followed up on. Traffic Bureau update (Officer Gaudet): On Tuesday, February 25, 2020, I made contact with the registered owner of MV2, Flexdrive. The representative from Flexdrive stated MV2 is registered to the company but Lyft uses their vehicles for ride sharing. I contacted Lyft and spoke with a party named Jonas from their support team. Jonas stated he would make contact with the operator of MV2 and ask them to contact me to give a statement. Lyft stated they would also conduct their own internal investigation. On Wednesday, February 26, 2020, I spoke with the operator of MV2, Gary Procaccini (S36066201). Procaccini stated he was operating a 2019 Santa Fe (NY Reg: T797800C) as a Lyft driver on the evening of February 22, 2020. Procaccini stated he was traveling on Washington Street (W) towards the Massachusetts Turnpike entrance in West Newton. Procaccini stated he was in the wrong lane and looked in his mirror before attempting to merge into the correct lane. Procaccini stated to the right of his vehicle he observed a vehicle "flying" towards the entrance to the Pike. Procaccini stated he came to stop to allow the vehicle to pass. Procaccini stated the vehicle passed at a high rate of speed and he continued on his way onto the Pike (E). Procaccini stated he had a passenger in the vehicle using Lyft servies and there were no signs that a crash had taken place. Procaccini's information has been added to his report to reflect him being the operator of MV2. I then spoke with Peter Houser who is a Driver Services Representative with Lyft. Houser stated he had just visually inspected the vehicle alleged to have been involved in the crash. Houser stated the vehicle was in "pristine" condition and he observed no damage to the front passenger side quarter panel of MV2. Houser e-mailed me pictures of the vehicle. I observed no damage to MV2. Those pictures were submitted to the IT Bureau.

On Tuesday, March 31st 2020, at approximately 5:11pm, I, Officer Brooks, responded to 580 Washington street for a motor vehicle accident. Upon arrival I spoke with the operator of MV1(MA REG 5TRL60) who stated he was pulling out of the parking lot of 580 Washington street, and was attempting to turn left to head westbound on Washington street. He stated he was already in the first eastbound lane of traffic when MV2( MA REG 1KL458) came at a high rate of speed eastbound on Washington street in the center lane and struck his vehicle. After striking his vehicle, MV2 hit MV3(MA REG7BG273) which was parked legally in front of 580 Washington street. The collision caused MV3 to hit MV4(MA REG 3AGN11) which was parked legally in front of MV3. MV1 had damage to the front passenger side bumper. I then spoke with the operator of MV2 who stated she was traveling eastbound on Washington street in the far right lane. She states MV1 pulled out of the parking lot of 580 Washington street and struck her vehicle causing her to loose control and hit MV3 which was parked. Her car had airbag deployment and damage to the passenger side rear wheel well as well as the front end. MV 3 had damage to the rear end and front end. MV4 had minor scratches to the rear bumper. Nobody was injured in the accident, and the operator of MV2 signed a refusal with Cataldo. Tody's towed MV2 due to the damage. A motor vehicle inventory was filled out and filed.

On Thursday 4/30/2020 at aprrox 1859hrs, while assigned to N-494, I responded to the intersection Washington St and Centre St for a 2 car MVA. There I observed MV2 stopped in the far left lane and MV1 stopped in the adjacent lane. They described the Jeep Wrangler as a "newer" model, possibly a four door with a hard top. The residents of 163 Suffolk Rd have a camera system and will view the footage at a later time. Units checked the area for the Jeep Wrangler but were unable to locate it. Todys's responded and removed MV1 from the scene. **Update by Ofc. Hamann** At 0600 hours, Dispatch advised me that a male party that was involved in an accident last night (5/15/20). Brett Catlin stated that he was driving home from CVS last night and admitted to looking down at his phone and then had crashed. He stated after he crashed he drove to his home, which is 4 houses down. He stated he had called the owner of the vehicle and apologized and stated he was going to report it in the morning. Operator 1 stated he sustained no injuries and was checked out by medic 3 in which he signed a patient refusal. MV1 sustained minor damage, scuffs, to the right side leather saddle bag. The northbound lane was coned off for more than a block due to a Feeney Brothers construction detail with Officer Edrehi manning the post. A backhoe, a bobcat and a generator all occupied the northbound lane that was clearly blocked off. Officer Edrehi stated that due to Feeney Brother construction detail Waverly Ave was diverted to 1 way traffic travelling northbound in the southbound lane. Officer Edrehi stated he observed Operator 1 following multiple vehicle and as he approached the intersection of Franklin St Operator 1 crossed over the marked lanes and past the cones and at a low speed fell into the construction hole. Due to the statements provided I issued Operator 1 MA Uniform Citation T1269531 for C89 S4A Marked Lanes violation (warning). Operator 1 was assisted in lifitng his motorcycle and drove off without further issues.

On 06/10/2020 at approximately 16:30 hours, Sgt. Lee (Unit 531) and I (Unit 523) responded to Waverley Avenue and Sargent Street for a two car motor vehicle accident that involved one vehicle rolling over and a possible head injury to that driver. The accident occurred at 16:11 hours on this date. Both streets are public ways and maintained by the City of Newton. On arrival, I observed a black Hyundai Elantra (MV1) bearing MA Reg.# 1DHN15 resting in the middle of the intersection facing eastbound on Sargent Street heading towards Kenrick Street. The vehicle had heavy front end damage. I then observed a gray Honda CRV (MV2) bearing MA Reg.# 8DZB10 rolled over resting on it's hood facing in a northeasterly direction in the southbound travel lane of Waverley Avenue just past the intersection. I observed the point of impact to be on the passenger side rear wheel area due to the damage which included a large dent above the tire well. Digital photos were taken of the scene and intersection. At this point the operator, Jose Valderrama (MA DL# S27590058), of MV2 was already transported to Beth Israel Hospital in Boston, MA for a possible head injury. The operator was also not aware of what occurred at the time by the responding officers. It was also determined by the initial responding officers (Sgt. Rooney, Ofc. Pohlman & Ofc. Anderson) that Mr. Valderrama driver's license was suspended in Massachusetts by NPD Dispatch. Next, I spoke to the operator of MV1 identified as Cora Lyden (ME DL# 8625330). Lyden states that she was traveling straight ahead (EB) on Sargent Street attempting to go straight through the intersection on to Kenrick Street. She says as she approached the intersection the signal light changed to yellow and she proceeded through when she suddenly saw MV2 coming at her. She then jammed on her brakes and then made contact with the other vehicle. She says that the other driver must have been speeding and went through the red light. Next, Sgt. Lee and I responded to Beth Israel Hospital for a follow up on Mr. Valderrama. The hospital staff stated that there were no signs of a head injury and that he was now coherent. He sustained road rash on his left arm and shoulder area due to the seatbelt and front air bag deployment. We then spoke to him and he stated that he was traveling straight ahead (SB) on Waverley Avenue at about 30 to 35 miles per hour. He says that his signal light was green when he attempted to go through the intersection and another motor vehicle made contact with him. After that, he does not recall what occurred. I asked him if he saw the other vehicle approaching and he stated that he did not see it. After at the station, I followed up with Dispatch and they stated that there was two people that called in to report the accident that they attempted to call back to see if they witnessed the event. They had since not returned their call. I then called both numbers and both parties stated that they were only notifying us and did not observe the accident occur. Due to conflicting statements by both operators and no witnesses on scene, I am unable to determine fault for the accident. Mr. Valderrama will be issued MA Uniform Citation #T2080466 for Ch. 90/S23/E Operating a Motor Vehicle after Suspension of License. Tody's towing responded and towed both vehicles. M/V inventories completed by me.

On 06/14/21 at approximately 12:00 hours, Sgt. Wade received an email from Nina Wang (D.P.W. Transportation Traffic Superintendent) that a city utility trailer with a signboard attached to it was struck by another motor vehicle at 275 Brookline Street sometime over the weekend between 06/11/21 and 06/14/21. A photo was sent over in the email of the utility trailer and signboard knocked over on it's side over the sidewalk. The utility trailer is 2020 Wanco color orange bearing MA Official registration # M9994A. The trailer was parked in the bicycle lane of the northbound travel lane next to 275 Brookline Street. The signboard was there in an attempt to get motorists to drive the speed limit due to the Memorial Spalding School located at 250 Brookline Street. On 06/14/21 at approximately 12:00 hours, I responded to Brookline Street to photograph the trailer and possibly process the scene. The trailer could not be located. I learned later that the trailer had already been towed by Tody's towing and brought to the City D.P.W. yard located at 110 Crafts Street. On 06/15/21 at approximately 09:00 hours, I went over to 110 Crafts Street and was told that the trailer/signboard was in the garage. The trailer was already being repaired to be placed back in to service. It had sustained damage to the two rear brackets where the jacks are located and the license plate bracket and registration plate were bent. The signboard message frame board is no longer aligned properly as a result of the accident but reportedly still functions. After, I then responded back to 275 Brookline Street and took digital photos of the scene. I observed in the bike lane at the accident scene two small pieces of lens covers to a vehicle with no identifying marks. I also did a canvass for possible surveillance cameras in the area. There was no response at number 269 and 275 Brookline Street but I left a note for them to call me. 283 Brookline Street has no surveillance cameras. Memorial Spalding School is across the street from the accident and I observed a camera on the side of the building which faces the staff parking lot on to Brookline Street. I spoke to a staff member Jesse Brackett at the school. He called David Stickney (Superintendent of Schools) and notified him of the accident. Mr. Stickney was provided with my email and will send over any possible footage if he has it. At approximately 12:45 hours, I spoke to Mike Sylvester who resides at 275 Brookline Street. He stated that he had surveillance footage and emailed it over to me. I viewed the footage which is a camera that views his approx. 75 foot driveway leading out to Brookline Street. There is a fence and bushes behind where the signboard is located which limits it's view. The video shows the accident took place on 06/11/21 at 18:48 hours. At this time, you can see the trailer is struck and slowly falls over on it's side. Immediately after a white colored four door sedan continues to slowly drive past the scene northbound on Brookline Street in the bicycle lane without stopping. He stated that he had footage of the accident but it did not reveal any other details besides the same white sedan striking the trailer.

On 08/09/2021 at 13:56 hours I responded to the intersection of California St at Adams St for a crash involving a motor vehicle and a bike. California St and Adams St are both public ways in the City of Newton. Upon my arrival I was informed the bicyclist , Joshua Cohen was transported to Newton Wellesley Hospital for treatment . I spoke with Marco Diaz who stated that he was driving his 2004 Honda Pilot ( MA reg 1YEM63 ) east bound on California St near Adams St. Diaz stated that as his vehicle passed Adams St he heard a loud bump coming from the rear passenger side of his vehicle. Diaz stated that he looked into his rearview mirror and realized that something had crashed into his vehicle. Diaz stated that he stopped his vehicle had saw a male on a bike had crash into his vehicle. Diaz stated that he was not injured in the crash and his vehicle did not require a tow. I observed small scrape marks on the rear passenger side quarter panel of Diaz's vehicle. Cohen's bike was a Metakoo 26'' electric bike, that has a top speed of 20 MPH. Cohen's bike did not appear to have any damage on it and was secured at Cohen's residence. I spoke with Cohen at the ER at Newton Wellesley Hospital were he was waiting treatment for a laceration to his forehead and a possible concussion. Cohen stated that he "was going to fast and took a too wide of a turn" from the north bound lane of Adams St onto the east bound lane of California St. Cohen stated "I hit into him " ( Diaz ) . Based upon statements made to me by Diaz and Cohen and my observations of the crash area I belive that Cohen is at fault for this crash. I took photos of Diaz's vehicle and Cohen's bike, the disk was downloaded by the NPD IT Bureau.

Operator of MV3 stated she observed the collision of MV1 and MV2 but she did not have enough time before MV1 struck her vehicle. Operator of MV3 stated she observe the operator of MV2 fail to come to a complete stop at the Stop Sign causing the accident. Medic 3, Medic 1, Ladder 3 and Engine 3 were on scene for the report of multiple injuries. The operator of MV1 and MV3 were transported to NWH with some minor injuries. MV1 and MV3 sustained heavy damage and were towed by Tody's. MV2 had minor damage and did not require a tow.

On 10/06/2021 while assigned to marked cruiser N491 at approximately 15:26 hours I responded to 527 Washington Street for a report of 2 car MVA. Newton FD requested EMS for evaluation of Driver 1 because she was "feeling dizzy and shaky" after the accident. Medic 1 cleared with a patient refusal. Upon arrival on scene, I spoke to Ms. Marcia Ruth Drector (Driver 1 of MV1), who stated that she was at intersection of Washington Street and Adams St waiting for green traffic light to continue driving straight forward / eastbound on Washington Street. She said that when the traffic light turned green she started going forward, when all of a sudden a light blue sedan (MV3) cut her way driving southbound from Adams St to Lewis Ter. Ms. Drector stated that she had to slam on the brakes to avoid collision with MV3, which was traveling at high rate of speed and drove just a few inches away from the front of her car after she stopped. At the same moment, another car (MV2), which was behind her car (MV1), crashed into the rear end of her car (MV1). Ms. Drector could not recall any specific details about MV3 or its driver/occupants, other than it was a light blue sedan. I then spoke to Mr. Stephen Achille (Driver 2 of MV2), who stated that he was facing eastbound in the left lane on Washington Street. Mr. Achille said that he started changing lanes because a car (MV4) in front of him was waiting for a left turn to Adams St. He stated that when he was getting into the right lane, MV1 suddenly stopped in front of him and he crashed into it. I advised all parties that there will be an accident report on file for insurance purposes.

MV2 operator stated he had exited Rt.95 northbound at the Grove St exit. MV2 operator said he was attempting to drive straight on to Recreation Rd/Rt.95 northbound on-ramp. MV2 operator said he stopped at the stop sign at Grove St and looked both ways. MV2 operator stated he did observe MV1 driving southbound on Grove St at a reasonable speed. However, MV2 operator said he thought MV1 had stopped to let him go. MV2 operator said as he attempted to drive straight onto Recreation Road/Rt.95 northbound on-ramp, he struck the driver side of MV1. MV2 operator reported no injuries at this time. On November 21st, 2021 at approximately 07:56 hours while working N491 I along with Officer Mike Boudreau responded with NFD and Medic One to a report of a MV crash with injuries in the area of Centre St @ Franklin St. On my arrival the Medics were evaluating two adult males who standing outside one of the involved vehicle from this crash. There appeared to be several involved vehicles to this crash however two of them had been parked prior to the crash. The vehicle which the two unidentified males were standing by was MA reg. 9HY939, a 1988 Black Ford F150 PU, listed registered owner was a Peter J. Jara-Castro, #28 Ridge Ave, Lynn Ma. Mr Castro was one of the two males standing by that truck. The above vehicle appeared to have been travelling N/B on Centre St when it skidded off the road, jumping the curb striking a 1 foot slate retaining wall to the property to #573 Centre St. It continued onto that property driving over shrubs, small trees, then onto the driveway(same address) which is actually on Newtonville Ave, hitting the homeowners 2007 Nissan Versa, Ma reg. 6BH269, forcing it into more shrubbery. The Truck continued across Newtonville Ave where it struck another parked vehicle, a Grey 2019 Volkswagon, Ma reg. 1NFC27 on the rear passenger side pushing it forward. The truck finally came to a stop after driving into a wooden fence to the property of #559 Centre St.(Fence is on Newtonville Ave.) I spoke with the owner of the truck after he was evaluated by the Medics, who spoke limited English. The owner of the truck Peter Jara-Castro finally said he was driving the truck prior to the crash. His statement was he was travelling N/B down Centre St when another vehicle came out from Newtonville Ave causing him to slam on his brakes. Mr Castro doesn't remember much after that, possibly from hitting his head. At this time I couldn't determine who the actual driver was. Both Mr Castro and the passenger, sustained injuries but both declined to go to the hospital. Both were later picked up by an UBER driver. A Firefighter heading to work reported seeing the truck pass him on Centre St N/B at high rate of speed but never saw the driver or the actual crash. Both property owners that sustained damage were given Trucks insurance info. Pictures were taken by both property owners. Vehicle #1 and vehicle #2 were towed from scene. Both sustained heavy damage. Vehicle #3 sustained minor damage I was unable to make contact with that owner at this time. However later in the day I was able to make contact with owner of vehicle #3. She was given all vehicle #1 information along with his insurance. A citation for speeding was later issued after investigation was completed and witness statement. Citation#t1444076 was later sent VIA mail.

On Wednesday 12/1/21 at approximately 0800 hours while assigned to marked unit n498 I was dispatched to 336 Winchester Street for a report of a single vehicle crash with no injuries. Upon arrival I observed a white BMW X3 partially off the road with heavy front end damage on the Northbound side of Winchester Street in front of 336. I spoke with the operator, PHILLIPS Illana, who states she was travelling Northbound on Winchester Street at approximately 30 MPH when she heard a pop and the car pulled immediately to the right and she felt an impact and the air bags deployed. It appears as though the impact she felt was the Utility Pole in front of 336 Winchester Street. Pole sustained significant damage, but still appeared to be safe. Verizon was notified to respond as soon as possible. Also on scene was STOLYAR Polina, who states that she was walking on the sidewalk toward her house at 345 Winchester Street. Polina states she observed the white BMW travelling at a normal speed Northbound and she also heard a pop and saw a quick flash that she described as a flame come from the passenger side tire of the vehicle causing it to swerve off the road and strike the Utility Pole. Polina stayed on scene to make sure PHILLIPS was ok and to speak with me. After speaking with the operator, PHILLIPS, and the witness, STOLYAR, it appears to me that the passenger side tire of the BMW blew causing the vehicle to veer off the road striking the Utility Pole. I walked Southbound down the Northbound side of Winchester Street to see if I could locate any sort of road defect that could have caused the tire to suffer a catastrophic malfunction. I was unable to locate any such defect. PHILLIPS mentioned to me that recently the light on her dashboard illuminated indicating low tire pressure and she brought the vehicle to have the air pressure refilled. She expressed a concern in hindsight that whomever added air may have over inflated the tire. Exact cause of tire malfunction is unknown at this time.

On 03/20/222, I responded to Washington Street @ Hovey St for a 2 car MV accident. Upon arrival I spoke with the operator of MV 1, who stated that she was on the left lane on Washington St waiting to turn left onto Hovey street. She stated that the oncoming vehicles stopped for her to turn left on Hovey St when MV2 swerved around the vehicles and crashed into her causing major damage to the center front and right side of the MV. The operator of MV1 stated that she had a dash cam that caught the whole accident on video and they she would provide the video so I can view it. I then spoke to the operator of MV2, who stated that he was driving in far right lane Westbound on Washington St when the operator of MV1 turned left and hit him causing major damage to the center front and left side of the vehicle. He stated that he had the right of way and that there were no cars in front him which is why he continued driving straight. He also stated that he did nothing wrong and that he was following the speed limit. The operator of MV1 was able to send the dash cam footage from the accident. Based on my observations the video was consistent with the statement that operator of MV1 gave. In the video it is clear that MV1 had the right of way to make the left turn onto Hovey St. The operator of MV2 was mailed Mass Uniform Citation T1445888 fir M.G.L 89 4A Marked Lanes violation. I am mailing the citation due to the fact the that operator of MV2 had left the scene already. Medics arrived on scene and a patient refusal was signed. Todys arrived on scene and took possession of both motor vehicles.

ON 6-9-22 AT APPROX. 1553HRS. WHILE WORKING N497 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT HIT AND RUN. UPON ARRIVAL AT 469 WOLCOTT ST. I SPOKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES HE WAS TRAVELING E-BOUND ON WOLCOTT ST. HE STATES VEHICLE #2 WAS BACKING OUT OF 469 WOLCOTT AT A HIGH RATE OF SPEED. HE STATES HE BEEPED TO MAKE HIS PRESENCE KNOWN BUT WAS STILL HIT IN THE LEFT FRONT QTR. PANEL BY VEHICLE #2. HE STATES AFTER THE CRASH VEHICLE #2 SPED OFF TOWARD LEXINGTON AT A HIGH RATE OF SPEED. DRIVER OF VEHICLE #1 STATES HE GOT THE PLATE NUMBER OF VEHICLE #2 ( MA REG : 225LZ2 ). AS OF THE WRITING OF THIS REPORT I HAVE BEEN UNABLE TO MAKE CONTACT WITH THE OWNER OF VEHICLE #2. VEHICLE #1 HAD MINOR LEFT FRONT END QTR PANEL DAMAGE. HE REPORTED NO INJURIES AND WAS ADVISED TO CONTACT HIS INSURANCE COMPANY. CLEARED WITHOUT FURTHER INCIDENT.

At approximately 1840HRs on Wednesday, June 08, 2022, I was dispatched to 125 Parker St. for a road rage incident. Upon arrival, I spoke with Derek Eggleton, who stated that he was walking from his truck on Glenwood Ave. to 125 Parker St. with building supplies in hand. As he crossed Parker St., he noticed a Maserati bearing MA Plate: SUGAR approaching him, southbound, at a high rate of speed. Derek then threw said building supplies and moved out of the way of the vehicle because he believed it was not going to stop. He then approached the vehicle which had slowed down and Derek states it "tapped" him in the leg. Heated words were exchanged between Derek and the operator of the vehicle, identified as Sidney Gersh, who ultimately drove off. While I was on scene, Officer Hatfield spoke with Sidney, who stated we was indeed involved in the incident, but states he did not hit Derek. Officer Hatfield observed no damage to the vehicle and I observed no marks on Derek, who signed a patient refusal. I advised Derek that I would be documenting this incident and explained to him that if he so chooses, he may seek charges through Newton District Court. He stated that he understood and would take that into consideration.

MV1, MV2, and MV3 were all driving EASTBOUND on Commonwealth Ave in the left turn only lane to turn onto Lexington St Northbound. All vehicles were stopped at a red light when a 4th vehicle struck MV3 with such force that MV3 collided with MV2 and MV2 collided with MV1. MV4 left the scene immediately after the collision. Drivers from all three vehicles were unable to give information on the vehicle that fled the scene speeding EASTBOUND on Commonwealth Ave. MV1 sustained minimal damage to the rear bumper. MV2 & MV3 sustained minimal damage to both front and rear bumpers. Passenger from MV2 was checked by medics for reports of whiplash and signed a refusal. All vehicles were driveable after the incident.

On Monday, July 11, 2022, while assigned to unit N492, I responded to the area of Commonwealth Avenue and Temple Street, Newton for a report of a crash with minor injuries. The weather at the time of the crash was clear and sunny. The road surface was dry. Commonwealth Avenue and Temple Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Ms. Rui Zhang (SA4940828). Ms. Zhang stated she was operating her 2016 Subaru Outback (1WRP35) on Commonwealth Avenue (W) towards Temple Street. Ms. Zhang stated she activated her right turn directional and was slowing down to take a right turn onto Temple Street. Ms. Zhang stated she began to take her right turn, a vehicle appeared on her right side and crashed into the front passenger side of her vehicle. Ms. Zhang her and her son Kenneth reported no injuries as a result of the crash. I observed damage to the front passenger side bumper/tire/fender area of MV1. Tody's responded and removed the vehicle from the roadway. I spoke with the operator of MV2, Mr. Thomas Conneely (S30459555) as he was being treated by Newton Medics. Mr. Conneely stated he was operating his 2019 Honda CRV (MA: 58252) on Commonwealth Avenue (W) towards Temple Street. Mr. Conneely stated he was traveling behind MV1 for some time and the vehicle was slowing down and speeding up down the length of Commonwealth Avenue. Mr. Conneely stated as they approached the Temple Street intersection, MV1 had stopped in the middle of the road and he believed it was taking a left turn on to Temple Street. Mr. Conneely stated he went to around the right side of the vehicle when it suddenly took a right turn into his vehicle as it was passing. Mr. Conneely said his vehicle then went airborne twice and he ended up in the yard of 1633 Commonwealth Avenue. Mr. Conneely and his passenger, Ms. Susan Conneely were transported to Newton Wellesley Hospital by Newton Medics for evaluation. Tody's towing reponded and removed MV2 from the property of 1633 Commonwealth Avenue. Ms. Nancy Ghabi stated she was operating her vehicle on Commonwealth Avenue (W) behind MV1 and MV2 at the time of the crash. Ms. Ghabi stated she observed MV2 to be traveling at a high rate of speed and observed it travel to the right of MV1 and try to go around it. Ms. Ghabi stated she two vehicles then crashed and MV2 ended up in the lawn of 1633 Commonwealth Avenue. Ms. Ghabi described the actions of MV2 to be "erratic". Mr. John Carey stated he witness the crashed as he was standing on the sidewalk in the area of 1633 Commonwealth Avenue. Mr. Carey stated MV1 was slowed down in the intersection and MV2 tried to go around MV1's passenger side. The vehicles then crashed and Mr. Carey believes MV2 did not attempt to brake post collision and MV2 ended up on the lawn of 1633 Commonwealth Avenue. Mr. Conneely will be cited with Massachusetts Uniform Citation 510670AB for Chapter 89, Section 4A (Unsafe Lane Change) and Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Pictures of the property damage to 1633 Commonwealth Avenue were taken and submitted to the IT Bureau.
