Narrative:
The operator of MV1 stated she was traveling Westbound on Washington St. at Church St. in the left travel lane and was coming to a stop behind several other cars at the red light. She stated as the light turned green she was struck from behind by MV2. As she proceeded to pull over, MV2 continued to travel Westbound on Washington St. and didn't stop. MV1 sustained minor damage to the right rear and was able to drive away from the scene safely. MV2 came back registered to GOMES, Leonel of 72 High St. Waltham, and dispatch requested Waltham PD respond to check for the vehicle. Waltham PD located the vehicle in the driveway and the registered owner was sitting in the vehicle. An ELMUS, Elmer was on scene to translate for the operator as he only spoke Spanish. Dispatch relayed to me that there was no status found on GOMES' license, and upon speaking to GOMES and ELMUS they confirmed he did not have a license or passport. GOMES relayed to ELMUS that he was aware that he struck MV1 at the intersection of Washington St. at Church St. He stated that the light was green and that he was traveling Westbound on Washington St behind MV1. He attempted to brake and turned his wheel to the right to avoid the crash, but struck MV1 in the right rear. He stated that he thought MV1 was going to leave so he didn't stop and drove back to his residence in Waltham. MV2 sustained minor damage to the front left bumper. All parties involved were uninjured.

On 2/13/2019 at approx 1240hrs while assigned to 497 I responded to the area of 1784 Beacon St for a report of a two car crash without injuries. Upon arrival i observed Ma Reg 9LT451 which had been operated by Frank BOOD angled in the roadway with front passenger side damage and Ma Reg 8AGA10 which had been operated by Harold SILVERSTEIN off the roadway lodged into several broken sections of wood fence on the property of 1784 Beacon St which Newton City Census shows as belonging to Elliot SURKIN. I spoke with BOOD who stated he had turned his right directional on , was slowing and turning into the driveway of 1784 Beacon St where he was working as a contractor when SILVERSTEIN came up on his right struck his front passenger side than drove into the fence. SILVERSTEIN stated he did not notice BOOD slow or his directional and tried to stop and turn but struck BOOD than the fence. Todys responded and towed SILVERSTEINS vehicle.

All operators state MV3 was stopped in traffic, with MV2 stopped in traffic behind it. Both vehicles were heading southbound on Centre St. MV1 failed to stop behind MV2, colliding with the rear of MV2. MV2 was sent forward from the impact and collided with MV3. No injuries were reported on scene and no tows were needed. On my arrival all parties had exchanged information. After further investigation it was determined the operator of MV2 was driving with a suspended license. I spoke to her via phone and she stated she thought it was cleared up. I advised her that I would be mailing a citation to her (Citation #T1441680) for operating after a suspended license. She was driven away from the scene by a neighbor (Michael Manna, S20209078).

On 6/6/20119 at approx 1228hrs while assigned to 497 I responded to the area of 587 Chestnut St for a report of a two car crash upon arrival I met with the operator of Ma. Reg 8187VV Teresa MALLOY who stated she was traveling NB on Chestnut St when the postman ( Musar from Waban Post Office 929-278-0647) stepped out in the street , MALLOY stopped and was hit from behind by Ma. Reg 98FT16 being operated by Mitchell TVERSKOY. I spoke with TVERSKOY who stated MALLOY stopped suddenly and he crashed into her. MALLOY'S passenger, Janet HARRIS was having pain after the crash and was transported to NWH by Cataldo Ambulance. TVERSKOY'S vehicle towed by TODYS.

On June 24, 2019 at approximately 17:12 hours I responded to a two car motor vehicle crash on Washington St at the intersection of Prospect St. Both vehicles were pulled over facing westbound on Washington St. MV1 had rear end damage, while MV 2 Had front end damage. Both operators said that the vehicle in front of MV1 made a sudden stop and turned left onto Prospect St without signaling, MV1 had to brake suddenly and MV2 crashed into the rear of MV1. The operator of MV1 complained of back pain but declined transport to the hospital after evaluation. MV2 was towed from the scene by Tody's.

At approximately 1740HRs on Tuesday, July, 23, 2019, I was dispatched to the intersection of Centre St. and Washington St. for a two vehicle crash. Upon arrival, I confirmed that all parties were not injured. The operator of MV1 (MA Pass: 375983) stated that as she traveled northbound on Center St., she stopped at the yield sign in the intersection, so she could enter Washington St. (eastbound). It was at this time MV2 struck MV1 in the rear. The operator of MV2 (MA Official: M94771) stated that as she traveled northbound, she observed MV1 stop at the intersection and began to enter Washington St. MV1 then stopped again and the two collided. I noted damage to MV1's rear bumper area and damage to MV2's front bumper area. Neither vehicle required a tow and left under their own power. I cleared without incident.

Operator of MV1 was traveling southbound on Centre Street approaching the intersection of Pleasant Street. He failed to apply the brakes in time to stop when he saw the operator of MV2 had stopped to allow another vehicle enter onto Centre Street from Pleasant Street. He estimated his speed at the time of the accident to be approximately 10 mph. Operator of MV2 was stopped on Centre Street allowing another driver pull out in front of her when she was struck from behind. Operator of MV2 complained of neck and back pain and was transported to NWH by Cataldo MED1. I observed very minimal damage to both the rear of MV2 and front of MV2. There were a few very minor scratches, but no other damage to either bumpers. The operator of MV1 and his passenger both signed patient refusals. Both vehicles were driveable and operator of MV2 was able to pull her vehicle into a parking spot on Centre Street.

On 09/19/2019, while on patrol in marked cruiser 499, I, Officer Gamble was dispatched to 63 Carl St for a report of a hit and run. Upon arrival, I noticed damage to the left rear side of a blue Toyota in the driveway. The reporting party, Doris Rui, told me she was operating her husbands vehicle (Mass Reg # 495DT4 Toyt Rav4 Blue). Doris was driving north on Centre St at approximately 1500 hours. She stated the traffic light turned green and she started to travel through the intersection of Centre Street and Walnut Street. Both streets are public ways in the City of Newton. At this time she felt a vehicle crash into the rear of her Toyota. Doris pulled over to the side of the road on Centre St to exchange information and examine the damage. Doris stated The owner/operator of the other vehicle (later identified as Jonathan Wolfe) left the scene of the accident without leave any of his information. I placed a phone call to Wolfe's cell but he did not pick up his phone. Video Clips captured from Doris phone of the accident were uploaded to this report. Vehicle 1 (Mass Reg # 1FSK61 Honda Civic) suffered major damage to front ride side of the vehicle. Vehicle 2 (Mass Reg # 495DT4 Toyota Rav4) suffered major damage to the rear bumper of the vehicle. The operator of Vehicle 1 (Jonathan Wolfe) was charged with S90/S24 Leaving The Scene Of Property Damage (See Incident Report # 19039626). Both vehicles have insurance, and there were no injuries.

On Monday, November 4th, 2019 at approximately 1602 hours I, Officer Newton and Officer Healy responded to a motor vehicle accident at 22 Randlett Park. Upon arrival we spoke to the operator of vehicle 1. He stated the accident occurred on Watertown st. He was stopped at the traffic light on Watertown St at Walnut St when he was rear ended by vehicle 2. He attempted to pull over and speak to the operator of vehicle 2 but vehicle 2 proceeded to keep driving westbound on Watertown St. Vehicle 1 followed vehicle 2 until vehicle 2 drove to his residence at 22 Randlett Pk. When speaking to the operator of vehicle 2 he seemed confused. He could not answer questions regarding the accident until he was asked multiple times. He stated he was driving from Newtonville to his residence. He stated that he did not realize he was involved in an accident so he proceeded to drive to his residence. It should be noted that in our observations of Mr. Usher, it did not appear that he had a chemical impairment at this time, although he displayed confusion, and shaky hands as we spoke to him. Mr. Usher's mother stated that he had been released from a rehab facility recently, and has "not been himself". As a result of my investigation, Mr Usher was issued Citation#T2079939 for a violation of M.G.L.Ch.90/24-Leaving the Scene Property Damage. Based on the totality of the circumstances, an Immediate Threat Request was faxed to the RMV, see Incident number 19047444.

On January 6th, 2020 at approximately 07:30 hours while working N491 I responded to a two car mv crash that occurred on Centre Ave at the Mass pike exit e/b side. On my arrival I spoke with driver #1 who stated he just came off the mass pike exit ramp onto Centre Ave when he was rear ended by mv #2. Driver #2 stated she was behind vehicle #1 on the exit ramp to the mass pike and when vehicle #1 pulled out into the intersection she followed him out at the same time looking to her left for on coming traffic not seeing that vehicle #1 had stopped in front of her causing her to crash into his rear drivers side. There were no injuries or tows for this crash.

On Friday, March 6, 2020 while assigned to Traffic unit N525, I responded to the area of the Countryside School (191 Dedham Street, Newton) for a report of a motor vehicle crash involving a City of Newton contracted school bus. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with the operator of MV1, Gianna Gemelli (S95586165). Gemelli stated she was traveling Southbound on Parker Street towards the Boylston Street (E) on ramp in a JSC Transportation owned 2016 Dodge Grand Caravan (MA SPN: 105641). JSC Transportation is contracted by the City of Newton to transport students through out the city. Gemelli stated as she was traveling on Parker Street, MV2 attempted to pass her on her driver side to take a left onto Boylston Street (E). Gemelli stated the front passenger side of MV2 made contact with the rear driver side of MV1. I observed minor damage to the rear driver side bumper area of MV1. Gemelli was transporting two students to the Countryside Elementary School at the time of the crash. The students in the vehicle were identified as Mayson Martinez and Hailey Nesvacil. No injuries were reported by any of the occupants of MV1. Countryside Principal Elizabeth Herlihy signed a patient refused for Mayson Martinez. Martinez's mother Rosa Mordan was notified of the crash by school staff. Gemelli signed a patient refusal for her daughter, Hailey Nesvacil. Regina Moody was notified of the crash. I spoke with the operator of MV2, Clary Binns (s100112165). Binns stated she was traveling Southbound on Parker Street towards the Boylston Street (E) on ramp. Binns stated she attempted to go around MV1 to enter the on ramp to proceed onto Boylston Street when the front passenger side of her vehicle made contact with the rear driver side of MV1. I observed minor damage to the front passenger side fender/wheel area of MV2. Binns reported no injuries. Prior to my arrival, Binns stated she had two passengers in her vehicle at the time of the crash that had left the scene. They were identified as Maya Bock and Jonatan Freedman-Naditch. I contacted parents for both parties to notify them of the crash. No injuries were reported. Binns, Bock, and Freedman-Naditch are all students the Meridian Academy in Boston. Clary Binns was provided with Massachusetts Uniform Citation T0115909 for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Photos where taken of both vehicles and submitted to the IT Bureau. Parker Street and Boyston Street are both public ways in the City of Newton.

The operator of MV1 states that he was traveling northbound on Winchester Street when he stopped in front of 170 Winchester Street to allow a pedestrian to cross the road. While stopped, MV1 was struck from behind by MV2. The operator of MV2 stated that he saw the MV1 stop for a pedestrian but did not have enough time to stop and subsequently crashed into MV1. Both vehicles sustained moderate damage and were towed from the scene (MV1 by Todys and MV2 by AAA). Both operators were evaluated by medics due to air bag deployment and signed patient refusals.

Vehicle 1 described as a 2019 Nissan Rogues color gray bearing MA Pan Reg 2SNN41 was stopped in traffic southbound on Parker St near the intersecting way of Hagen Rd. Both Parker St and Hagen Rd are public ways in the City of Newton. Vehicle 1 was then rear ended by Vehicle 2 described as a 2018 Ford Van color white bearing MA Con Reg S35168 which was traveling southbound on Parker St. As a result of the impact Vehicle 1 sustained damage to the passenger side rear bumper and quarter panel. Vehicle 2 sustained damage to the drivers side front bumper and fender. The operator of Vehicle 1 identified as Chelsea Halsor stated that she was stopped on Parker St due to another uninvolved vehicle which was stopped in front of her vehicle due to a pedestrian crossing Parker St in a crosswalk. Halsor stated that while stopped she was struck from behind by Vehicle 2. Halsor stated that the force of the impact caused her to hit her head on the steering wheel. Fire and Medics responded to the scene and evaluated Halsor. Halsor was released with a patient refusal. The operator of Vehicle 2 identified as Christopher Benoit stated that he was traveling southbound on Parker St and struck Vehicle 1 which was stopped in traffic. Benoit stated that he tried to avoid the collision by swerving to the right but was unable to and as a result struck the passenger side rear of Vehicle 1. Benoit stated he was uninjured. No other witnesses on scene. Vehicle 1 was driven from the scene by Halsor. An RMV check of Vehicle 2 revealed that the registration was expired as of 12/31/2019. Tody's responded and removed the vehicle. A Towed MV Inventory Sheet was completed. An RMV check also revealed that Benoits MA Drivers License (MA DL# S9550481) was suspended. As a result Benoit was issued in hand MA Uniform Citation T1268483 and cited for Mgl 90/23 Operation of a Motor Vehicle After License Suspension, Mgl 90/9 Operating an Unregistered Motor Vehicle on a Public Way and City Ordinance Violation 19-71 Following to Close.

On 6/27/20 at approx 1719 I responded to the area of Brookline ST and Hartman Rd for a report of a MVA with no injuries. Upon arrival I spoke with both operators involved in the accident. Operator of MV#1 stated she was traveling SB on Brookline St when she came to a stop to let a vehicle turn out of Spaulding Ln and then a pedestrian cross the crosswalk when she was rear ended by MV#2. Operator of MV#1 sustained no injuries and MV#1 had moderate rear end damage. I then spoke to operator of MV#2 who stated she was also driving SB on Brookline St when MV#1 suddenly stopped in front of her and she couldn't stop in time. She rear ended MV#1. MV#2 had heavy front end damage. The operator of MV#2 sustained no injuries. Bother operators signed patient refusals with the medics and both vehicles were able to be driven away.

On 07/15/2020, while assigned to N494, I, Officer Conary, responded to the intersection of Centre Street at Church Street for a motor vehicle accident. Upon arrival, I spoke with Operator of MV1 who stated that he was driving Southbound on Centre Street when the car in front of him stopped suddenly at the green signal. At this time, MV1 hit MV2 rear. Operator of MV1 said that he believes she was distracted with a hand held device. Operator of MV2 explained to me that she at the intersection of Centre Street and Church Street traveling Southbound. She got the green signal and she let the car traveling Northbound take a left turn onto Church Street before she continued through the intersection. At this time, MV2 was hit by MV1 from the rear. She stated she was not using her phone. It should be noted that the signal traveling Northbound on Centre is a green left signal to turn onto Church Street. After the green arrow, cars yield to oncoming traffic to turn. Fire and Medics arrived on scene and all parties involved signed patient refusals. MV1 was towed by AAA. MV1 had minor damage to the front bumper. MV2 was able to be driven from scene with minor damage to the rear bumper. No further incident to report.

Vehicle 1 was stopped in traffic on Commonwealth Ave at the intersection of Washington St. Both Commonwealth Ave and Washington St are public ways in the City of Newton. Vehicle 1 was located in the left turn lane of the westbound side of Commonwealth Ave. Vehicle 2 was located westbound side of Commonwealth Ave in the left turn lane. Vehicle 2 was located directly behind Vehicle 1. Vehicle 2 then moved forward colliding with the rear of Vehicle 1. Vehicle 2 then drove around Vehicle 1 and made a left hand turn on to Washington St from the center lane of Commonwealth Ave. Vehicle 2 left the area without leaving any identifying information. As a result of the impact Vehicle 1 sustained damage to the rear bumper. Since Vehicle 2 left the scene the damage to Vehicle 2 is unknown. No injuries were reported on scene, no transports were required. No tows were required. The operator of Vehicle 1 identified as Borris Partensky stated that he was stopped in traffic in the left turn only lane of Commonwealth Ave westbound awaiting the red light. Partensky stated that he was then rear ended by Vehicle 2. Partensky stated that he then got out of this vehicle and signaled to the operator of Vehicle 2 to stop. Partensky stated that Vehicle 2 did not stop, quickly drove around Vehicle 1 and made a left hand turn from the center lane of Commonwealth Ave on to Washington St. Partensky described the operator of Vehicle 2 as a white male in his early 20's with dark hair. Partensky described Vehicle 2 as a newer model Chevrolet Silverado Pickup color white with a paper temporary license plate. Partensky states that the license plate may have started with the numbers "33" and ended with "IT". The state of the license plate is unknown. Partensky stated that his vehicle is a Tesla which has the ability to take photographs. Partensky states he will download the images and send them to me. I also spoke to witness Deb Finamore. Finamore states that she is a city of Newton employee and at the time of the crash was operating her personal vehicle (Ford Escape color Black).Finamore also stated that prior to the crash between Vehicle 1 and Vehicle 2 she observed Vehicle 2 tailgating her vehicle as she was traveling westbound on Commonwealth Ave. Finamore stated that Vehicle 2 tried to pass her several times on Commonwealth Ave and eventually made a left turn off of Commonwealth Ave on to Temple St. Finamore states that she was stopped in traffic at the intersection of Commonwealth Ave and Washington St and was located directly in front of Vehicle 1. Finamore states that she then observed Vehicle 2 located behind Vehicle 1. Finamore states that she that when she got a green signal she made a left turn onto Washington St and heard the impact of the collision between Vehicle 1 and Vehicle 2. Finamore states that she then observed in her rear view mirror Vehicle 2 drive around Vehicle 1 and make a left turn southbound onto Washington St. Finamore states that she attempted to get the registration information of Vehicle 2 however Vehicle 2 continued southbound on Washington St at a high rate of speed. Finamore states that she then followed Vehicle 2 at a safe distance and observed Vehicle 2 commit a red light violation at the intersection of Washington St and the entrance of Newton Wellesley Hospital (2014 Washington St). Finamore states that Vehicle 2 went around stopped traffic at the intersection by entering the left turn only lane to pass stopped traffic to the right. Finamore states that she then observed Vehicle 2 commit another red light violation at the intersection of Beacon St and Washington St. Finamore states that Vehicle 2 entered the left turn only lane at the intersection to pass stopped traffic to the right. Finamore states that she then lost sight of Vehicle 2 as it continued southbound on Washington St. Finamore described Vehicle 2 as a newer model Chevrolet pickup truck color white. Finamore believes that Vehicle 2 had a paper temporary plate, unknown registration number or state of registration. Finamore described the operator of Vehicle 2 as a young white male in his 20's. On 08/03/2020 I was able to view photo's of Vehicle 2 taken by witness Deb Finamore. The photo's revealed Vehicle 2's rear registration plate as 333991T. An NCIC check revealed that this registration belongs to a 2019 Chevrolet Silverado color white out of Maryland. The Maryland RMV also lists the vehicle registration as stolen. No further information was provided by the Maryland RMV. In the photo's Vehicle 2 also appears to be a 2019 Chevrolet Silverado color white. At this time I am unable to identify the operator of Vehicle 2. I called and notified the owner of Vehicle 1 Borris Partensky of my findings. Partensky will be providing this information to his insurance company.

On August 26th, 2020 at approximately 10:54 hours while working N491 I responded to the intersection of Centre St @ Centre Ave(both public ways) for a report of a two car MV crash. On my arrival I first spoke with operator #1 who reported stopped at the intersection of Centre St @ Centre Ave. She then began to proceed out onto Centre Ave but had to stop again due to oncoming E/B traffic on Centre Ave. She then reported getting rear ended by vehicle #2. Operator #2 who was from Brazil and operating on her Brazilian driver license had to have an interpreter( Portuguese) who stated she was behind vehicle #1 N/B on Centre St, began to follow #1 vehicle out into the intersection when vehicle #1 suddenly stopped again. She was unable to stop and crashed into the rear of vehicle #1. There were no injuries and neither vehicle needed to be towed.

On Tuesday, November 24th 2020 at 1733 hours, myself, N-496 R March and Sgt Selig responded to 1616 Centre St for a 3 car motor vehicle accident. Newton Dispatch was informed there were people injured on scene, but upon our arrival we observed no parties involved were injured. Medics checked all parties involved and patient refusals were signed. The first vehicle involved was (Mass Reg #5HW126, PORS CAYENN COLOR GRAY, operated by Michael Queen. Commerce Insurance). The second vehicle involved was (Mass Reg #7ZA446, 2004 HOND ACCORD, COLOR GREEN, operated by Guy Levin. Commerce Insurance). The third vehicle involved was (Mass Reg # 5HM522, 2018 VOLK ATLAS COLOR RED. Operated by Alan Kurd. Bankers Standard Insurance.) Driver 1 stated he was traveling south on Centre St when Driver 2 slammed on the brakes in front of him. Driver 1 stated he rear ended Driver 2 as a result. Driver 2 stated his vehicle was nearly at a stop on Centre St because of heavy traffic. Driver 2 stated Driver 1 collided into the rear end of his vehicle. Driver 2 stated the impact of the collision caused his vehicle to rear end Driver 3 in front of him. Driver 3 stated he was nearly at a stop on Centre St because of heavy traffic. Driver 3 stated Driver 2 collided into the rear end of his vehicle. All three vehicles suffered major damage. Todys' Towing came and took Vehicle's 1 and 2. Vehicle 3 was able to be driven from the scene.

On 12/8/20 I was working N492 when I responded to Crafts St. at Albermale Rd. for a report of a 2 car MVA with minor injuries. Upon arrival I spoke with the Operator of Veh 1 Megan Stekla. Stekla stated she was traveling Eastbound on Crafts St. when a Vehicle pulled out from Albermale Rd. to cross Crafts St. causing the car in front of her (Veh 2) to slam on the breaks, causing her to collide with Veh 2. I then spoke with the occupants of Veh 2 Brandon Alvarado and Sofia Alvarado. Brandon stated he was traveling Eastbound on Crafts St. when a vehicle attempted to cross over Crafts onto Albermale Rd. almost causing him to strike that vehicle. Brandon stated he slammed on his breaks to avoid the collision and was struck from behind by Veh 1. Sofia stated she had recently gotten a concussion and did not feel well. Fire and medics responded and Sofia was transported to NWH. The vehicles had minor damage and were able to be driven from the scene. Negative results on any description of the vehicle that cut across Crafts St. Both operators were provided a crash number.

On January 30th, 2021 at approximately 08:51 hours while assigned to N491 I along with NFD and Fallon Ambulance responded to the intersection of Center St @ Center Ave for a report of a two vehicle MV crash. On my arrival I identified operator of MV #1 as Elis Jamilet Perez Castillo. She reported first being on Center St N/B @ Center Ave stopped. She observed MV #2 who was in front of her on Center St proceed out into the intersection. She then also proceeded out into the intersection while at the same time attempting to change into another lane(go around MV#2) when MV#2 suddenly slowed in front of her. She attempted to brake but hit her gas pedal instead causing her to crash into the rear of MV#2. I then identified the operator to MV#2, Jaquan Coleman. He reported that he was entering out into the intersection of Center Ave E/B from Center St N/B when he was rear ended by vehicle #1. Both vehicles sustained significant damage. MV #1 was towed by Todys. Both signed patient refusals for Fallon Ambulance.

On March 16th, 2021 at approximately 08:56 hours while working N491 I responded to a report of a two car MV crash at the intersection of Centre St @ Centre Ave. On my arrival I located both involved vehicles. Vehicle #1 was a 2016 blue Toyota camry, MA tag 7HL412. The operator was identified as Renee Joseph. She stated she was on Centre St N/B and was attempting to merge out onto Centre Ave E/B when she was rear ended by vehicle #2. She reported having head and neck pain from the crash. However she declined medical attention at this time. Vehicle #2 was a 2020Toyota prius, MA tag 8ZZ382. The operator was identified as Jacob A Walker. He stated he was stopped behind vehicle #1 at the intersection of Centre St @ Centre Ave. As vehicle #1 began to enter out into the intersection he looked back to his left for oncoming traffic not realizing vehicle #1 has stopped again in front of him. He then crashed into the rear of vehicle #1. No tows were required from this crash. Operator #1 reported an injury but declined an Ambulance.

On Friday, March 19, 2021 while assigned to Traffic unit N525, I responded to the intersection of Boylston Street (E) and Walnut Street for a report of a crash involving a City of Newton owned vehicle. Boylston Street and Walnut Street are both public ways in the City of Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with the operator of MV1, Ms. Lizet Trabelsi (S07390016). Ms. Trabelsi stated she was operating her 2013 Infinity JX35 (MA: 8SXE60) eastbound at the Boylston Street (E) on-ramp near Walnut Street. Ms. Trabelsi stated as she was merging onto Boylston Street from her stopped position, a vehicle traveling on Boylston Street (E) abruptly took a right turn in front of her and continued to travel onto Walnut Street (S). Ms. Trabelsi stated she applied her brakes to avoid a collusion. At this time, MV2's front bumper area crashed into the rear of her vehicle. Ms. Trabelsi reported no injuries. I observed minor damage to the rear driver side bumper area of MV1. I then spoke with the operator of MV2, Mr. Paul Whelan (S63212595). Mr. Whelan is an employee of the City of Newton Water Department and was operating a City of Newton owned 2016 Ford Escape (MA MVN: M2530A). Mr. Whelan stated he was stopped behind MV1 waiting to merge onto Boylston Street (E). Mr. Whelan stated as MV1 began to merge onto Boylston Street, a vehicle already traveling on Boylston Street (E) turned off onto Walnut Street in front of MV1, causing them to brake quickly. Mr. Whelan stated the front bumper area made contact with the rear bumper area of MV1. Mr. Whelan stated he was traveling at a very low speed. Mr. Whelan reported no injuries. Photos of both vehicles were taken and submitted to the IT Bureau.

Both parties were evaluated by the medics and signed a refusal. MV1 sustained damage to the rear bumper and trunk. Operator of MV1 was able to drive her car after the crash. MV2 sustained damage to the front bumper and hood. Operator of MV2 called for a private tow.

On my arrival with the NFD I located both vehicles. First was a 2013 Subaru station wagon impreza, ma reg 5LMZ60 with heavy front end damage. The operator was identified as Ann Maria Harle. She reported going S/B on Centre St by Newtonville Ave when vehicle #2 suddenly stopped in front of her. She attempted to stop before crashing into the rear of vehicle#2. Vehicle #2 was a 2020 Jeep Cherokee, MI reg. EHJ9616 with rear end damage. Operator was identified as Lauren Duche. She reported going S/B on Centre St when a vehicle in front of he suddenly stopped due to a pedestrian crossing the street. She was then rear ended by vehicle #1.(cid:31)Vehicle #2 never made contact with the vehicle in front of her after being rear ended. Vehicle #1 was towed by Todys Garage Vehicle #2 was able to drive away. Fallon Ambulance check out all parties from both vehicles but all adults signed patient refusals.

He then rear ended vehicle #2 after stopping again in front of him while he was still looking to his left. Vehicle #2 was a black 2015 Hyundai genesis, MA plate 1FKC71, operator identified as Jeannie chanyang Jung. She reported that she was stopped N/B on Centre St @ Centre Ave. She then began to proceedr out into the intersection of Centre Ave when she had to stop due to oncoming traffic coming E/B on Centre Ave. She was then rear ended by vehicle #1. Operator to vehicle #2 was checked out by the medics due to being pregnant but signed a patient refusal. Neither vehicle needed a tow.

On 5/25/21 I was working N499 when I responded to the area of 7 Spaulding Ln for a report of an MVA with no injuries. Upon arrival I spoke with the operator of Veh 2 first ( Frank Desantis). Desantis stated that he was traveling southbound on Brookline St. when the car infront of him abruptly stoped because someone was turning. Desantis stated he was then struck from behind by Mv 1. Desantis also stated that he belived he had made contact with the vehicle in front of him after he was stuck however that vehicle did not stop and left the area, Desantis did not observe any front end damage. I then spoke with the operator of MV 1 Yajaira Matus who stated that she was traveling southobound on Brookline St. in traffic when the car in front of her abruptly stoped causing her to rear end it. Matus vehicle suffered heavy front end damage and was not driveable. Matus called for her own tow and the vehicle was parked on the side of the rd while she waited for it. Both patries stated they were uninjuried and were advised and provided an accident report number.

On Monday 8/23/2021 at approx 1420hrs, while assigned to N498. I responded to the intersection of Wells Ave and Nahanton St (public ways) in Newton for a two car MVA hit/run. Upon arrival I spoke with the operator of MV1, Sebastian Gallo, who stated that prior to the accident, he was stopped and waiting to take a right onto Nahanton St SB. He said the light was red and there were many cars preventing him from entering traffic safely. Gallo sated that the MV2, which was directly behind him, kept honking their horn. He said he moved his vehicle further, stopped again but was then struck from behind by MV2. Gallo said that MV2 then went around his vehicle, turned left onto Nahanton St(NB) and left the scene. Gallo's father, John Gallo was able to take a picture of MV2 license plate at the scene. MV2 had a MA reg plate of 5ZP590 which cam back to Geraldine Goldstein out of 677 Winchester St in Newton(The Coleman House). See incident report #21036212 No Injuries were reported.

On Wednesday, September 1, 2021, while assigned to marked unit N525, I responded to the area of 1488 Washington Street, Newton for a report of a motor vehicle crash involving a school bus. The weather at the time of the crash was cloudy was light rain. The road surface was wet. Washington Street is a public way maintained by the City of Newton. The operator of MV1, Fanes Nerestant (S47503048) stated he was operating a City of Boston owned 2012 Chevrolet School Bus (MA SBN: SB37810) west on Washington Street towards the Learning Prep. Mr. Nerestant stated he slowed his vehicle down and put on his right direction to take a right turn into the parking lot at 1488 Washington Street to pick up students at the Learning Prep School. Mr. Nerestant stated he then felt impact from the back of the bus and heard a "boom". Mr. Nerestant was transported to Newton Wellesley Hospital for neck and back pain. I observed minor damage to the rear driver side bumper/fender area. There were no other occupants on the bus at the time of the crash. The operator of MV2, Mr. Allen Simon (S44212695) stated he was operating his 2018 Nissan Altima (MA: CS12BM) west on Washington Street near 1488 Washington Street. Mr. Simon stated the school bus in front of him in his travel lane abruptly stopped to make a turn and he was unable to avoid crashing into the vehicle. MV2's front passenger side front end sustained heavy damage as a result of the collison. Mr. Simon reported no injuries. MV2 was removed from the roadway by Tody's towing. Mr. Simon stated his vehicle had a camera in it at the time of the crash and Mr. Simon stated he would forward the footage of the crash to me.

She stated that she was travelling N/B on Craft St when a vehicle in front of her suddenly stopped. She was able to stop her vehicle but was rear ended by vehicle #2. She later learned that vehicle #2 was forced into her vehicle after she was rear ended by vehicle #1. There were no injuries or tows due to this crash.

The operator of MV1, Mr. John Doughty (S80542461), stated to Officer Durickas he was operating his 2014 GMC 1500 (MA: 6DJ399) near 277 Elliot Street (N). Mr. Doughty stated a vehicle in front of him came to quick stop in the roadway and was attempting to make a left turn into the Stone Institute located at 277 Elliot Street. Mr. Doughty stated at this time he was coming to a stop and the vehicle behind him, MV2, rear ended him. Mr. Doughty reported back pain as a result of the crash and was transported to Newton Wellesley Hospital by Fallon Medics for further evaluation. Officer Durickas observed damage to the rear bumper area of MV1. A private tow was contacted by Mr. Doughty and his vehicle was removed from the roadway. The operator of MV2, Mr. Denis Doucet (S15405545), stated to Officer Durickas he was operating a City of Newton owned 2018 Ford F350 (MA MVN: M2954A near 277 Elliot Street (N). Mr. Doucet stated the vehicle in front of him came to a quick stop and the front end of his vehicle crashed into the rear of MV1. Mr. Doucet reported no injuries. Officer Durickas observed damage to the front bumper/front passenger side fender area of MV2. Mr. Doucet is employee d by the City of Newton DPW. Photos were taken of both vehicles and submitted to the IT Bureau.

Ms. Brown stated she slowed down in traffic to allow a vehicle in front of her make a turn. Ms. Brown stated MV 2 then crashed into the rear of her vehicle. I observed significant damage to the rear bumper/trunk area of MV1. Due to the damage to her vehicle, Ms. Brown was also advised to replace the two car seats in her vehicle. Ms. Brown reported no injuries on scene. The operator of MV2, Mark Whooten (S39870664), stated he was operating a City of Newton owned Ford F-350 (MA MVN: M99235) on Walnut Street (N) past the Beacon Street intersection. Mr. Whooten stated the vehicle in front of him abruptly stopped in their travel lane to allow a vehicle to turn. The front of MV2 crashed into the rear of MV1. I observed minor damage to the front area of MV2. Mr. Whooten reported no injuries on scene. Mr. Whooten is an employee of the City of Newton DPW. Photos were taken and submitted to the IT Bureau.

On Wednesday, February 23, 2022, while assigned to Traffic unit N525, I responded to the intersection of Washington Street and Elm Street, Newton with N492 for a report of a minor motor vehicle crash. Washington Street and Elm Street are both public ways maintained by the City of Newton. The weather at the time of the crash was clear and sunny. The road surface at the time of the crash was dry. I spoke with the operator of MV1, Mr. Kumar Bhol (S19441804). Mr. Bhol stated his vehicle (2018 Toyota Rav-4 (MA: 8HH159) was parked unoccupied on Elm Street next to the Village Bank in a metered spot. Mr. Bhol stated when he returned to his vehicle after leaving the gym, he observed another vehicle illegally parked behind his vehicle with it's front bumper very close to the rear of his vehicle. Mr. Bhol stated he observed minor fresh damage to the bumper area of his vehicle at this time. Mr. Bhol stated the owner of MV2 returned to her vehicle at this time and he attempted to exchange information with her. Mr. Bhol stated the owner did not want to exchange information and attempted to leave the area. Mr. Bhol stated he positioned himself in front of her vehicle in the roadway as it was leaving to prevent MV2 from leaving. Mr. Bhol stated at this time MV2 made light contact with his knee. Mr. Bhol stated he has no injuries as a result of the contact. Mr. Bhol declined my request to have him evaluated by Newton Medics. Mr. Bhol was also advised to not position himself in front of a moving vehicle to try and stop them from leaving the area. I observed very minor damage to the rear bumper area of MV1. It is inconclusive if the damage is fresh from this crash. I did observe obvious old damage to a sensor on the rear bumper of MV2 with black tape over it. I spoke with the operator of MV2, Ms. Jennifer North (S41819258). Ms. North stated she parked her 2015 Jeep Grand Cherokee (MA: 2XR747) behind MV1 on Elm Street in what she thought was a legal parking space. Ms. North stated when she returned to her vehicle, the owner of MV1 alleged the damage on his vehicle was caused by her. Ms. North stated she did not make contact with MV1 and attempted to leave the area. Ms. North stated Mr. Bhol would not let her leave so they contacted the Newton Police to assist. A query of Ms. North's license status showed it was expired/non renewable. I advised Ms. North of her license status and stated she could not drive her vehicle away from the area. Ms. North stated she understood and walked to her residence with her two dogs. Ms. North's vehicle was left legally parked in a metered parking spot on Elm Street. I observed no noticeable damage to the front bumper area of MV2. Ms. North report no injuries. As a result of Ms. North's license status she was issued Massachusetts Uniform Citation 306986AB in hand for Chapter 90, Section 10 (Unlicensed Operation) and Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care). It is unclear is the damage to MV1's rear bumper was caused by MV2.

On 3/5/22 at approximately 1610 hours, I responded to 2345 Commonwealth Ave for a 2 car accident. On scene I spoke with MV1 who stated that he was driving westbound on Commonwealth Ave when he was rear ended by MV2. He stated that MV2 was driving very fast and he was behind a truck causing him to brake. MV2 stated she was driving westbound and MV1 was continuously braking and she then attempted to switch lanes. As she was switching lanes, she stated MV1 braked and she had hit the right rear part of his bumper. No injuries were reported. Both vehicles had damaged but no tow was needed.

On March 18, 2022 at approximately 16:21 hours I, Officer Guarino, responded to the area of Commonwealth ave for a two car motor vehicle crash. Upon arrival, I observed both occupants out of their vehicles. MV2 had struck the rear of MV1 and were still together at the time of my arrival. The operator of MV1 had stopped for a pedestrian in the crosswalk and the operator of MV2 did not stop in time and crashed into the rear of MV1. There were no reported injuries and both vehicles were able to be driven away. Both parties exchanged information and were informed that a police report would be on file.

Both Vehicles were traveling Northbound on Centre Street when they were approaching the Washington Street intersection a vehicle entered intersection causing Veh # 1 to slam on brakes. Vehicle # 2 tried to turn into other lane to stop crash but was unable to brake in time. The traffic lights at intersection were not working and all flashing yellow or red. Which may have been why the vehicle entered the intersection from Washington Street. Vehicle #2 towed. No injuries.

On March 28th, 2022 at approximately 09:13 hours while assigned to N491 I responded to the intersection of Washington St @ Jackson Rd for a reported MV crash. On my arrival I located both involved vehicle E/B on Washington St @ Jackson Rd. Vehicle #1 was a 2019 Black Tesla X SUV, Ma reg. EV502N, operated by Jeffrey Cook. He stated he had just stopped on Washington St E/B due to the vehicle in front of him stopping by Jackson Rd. His vehicle was then rear ended by vehicle #2. Vehicle #2 was a 2020 Black Chev. Silverado PU, Ma reg. 2NBN74, operated by David Haber. HE stated he was behind vehicle #1 on Washington St E/B when vehicle #1 suddenly stopped in front of him causing him to crash into the rear of the vehicle. Vehicle #1 sustained heavy rear end damage. Vehicle #2 sustained heavy front end damage. Neither vehicle required a tow and no reported injuries at this time.

On April 8th, 2022 at approximately 07:07 hours while assigned to N491 I responded to a report of a MV crash at the intersection of Centre St @ Centre Ave. On my arrival to the scene I located both involved vehicles on Centre Ave just east of Centre St. Vehicle #1 was a black 2020 Honda Civic, Ma. plate2FGD71, operated by Sharon V. Torres. She stated she was stopped at the intersection of Centre St @ Centre Ave when she was rear ended by vehicle #2. Vehicle #2 was a black 2016 Mercedes sedan, Ma. plate 5TN334, operated by Jenasi s Bello. She stated she was stopped behind vehicle #1 N/B on Centre St @ Centre Ave. She further stated vehicle #1 began to proceed out onto Centre Ave so she began to follow behind, simultaneously looking to her left for oncoming traffic coming E/B on Centre Ave. She then struck vehicle #1 who had stopped again in front of her.

On April 8th 2022, at approximately 14:24 hours while assigned to N491 i responded to #495 California St for a report of a MV crash. On my arrival I located both involved vehicles just west of #495 California St. Vehicle #1 was a 2022 White Range Rover, Ma. reg. 3HML10, operated by a Nasim Lalehzarian. She stated she was going W/B on California St when she came to a stop due to a vehicle in front of her was stopped waiting to make a left turn onto Melville Ave. She was then rear ended by vehicle #2. Vehicle #2 was a Red Honda Accord Ma. plate 184-D10, operated by Michael Paniagua. He stated he was going W/B on California St when he observed the vehicle in front of him stopped ahead. He then applied his brakes but they didn't allow him to stop in time. He then crashed into the rear of vehicle #1. There was a possibility of an issue with the Hondas braking. Vehicle #2 sustained heavy front end damage and required a tow. There was no reported injuries.

SHE STARTED SHE STARTED BEEPING HER HORN REPEATEDLY AT WHICH POINT VEHICLE #2 STOPPED HIS VEHICLE ABRUPTLY AND SHE WAS UNABLE TO AVOID HITTING HIM. DRIVER STATES VEHICLE #2 STOPPED DELIBERATELY KNOWING SHE WOULD BE UNABLE TO AVOID HITTING HIM. DRIVER OF VEHICLE #2 STATES HE ENTERED HIGHLAND ST. FROM TEMPLE AND WAS TRAVELING S-BOUND. HE STATES VEHICLE #1 WAS BEEPING HER HORN REPEATEDLY. HE STATES HE STOPPED TO SEE IF THERE WERE A PROBLEM AT WHICH TIME VEHICLE #1 HIT HIS VEHICLE. VEHICLE #1 HAD MINOR FRONT END DAMAGE. VEHICLE #2 HAD MINOR REAR END DAMAGE. BOTH PARTIES REPORTED NO INJURIES AND SIGNED PATIENT REFUSALS. CLEARED WITHOUT FURTHER INCIDENT.

On Tuesday 5/3/22 at approximately 0934 hours while assigned to marked unit n498 I was dispatched to the area of Rachel Rd and Upland Ave for a report of a minor motor vehicle crash. Upon arrival I spoke with the Operator, LUNA CARDOZO Ana Regina, of motor vehicle #1 (ma reg 53D960). She states she was travelling northbound on Upland Ave when she stopped in front of 355 Upland Ave. to let someone cross the street. She states as she was stopped she was struck in the rear by motor vehicle #2. Operator, LOUGHLIN William, of motor vehicle #2 (ma Reg 9470MR) states he was travelling behind motor vehicle #1 and mis judged the distance between the two vehicles and could not stop in time when motor vehicle #1 stopped to allow someone to cross the street. While interviewing both parties i obtained information that the operator of motor vehicle #1 did not have a drivers license. I Informed LUNA CORDOZO that she was going to be cited and issued Massachusetts Uniform Citation T2012916 c90 s10 Operating Without a DriversLicense. The vehicle was parked legally on Upland Ave and LUNA CORDOZO's husband, EIDERICK-COSTRA Wagner, a licensed driver, would be coming later in the day to pick up the vehicle.

On 5/20/2022 at approx 0825hrs while assigned to 497 I responded to the area of Washington St and Quinobequin Rd for a report of a two car crash with one operator being pregnant. Upon arrival I met with Marlow Hansen who was operating Ma Reg FRMGRL who stated she was taking the right onto Washington St from Quinobequin Rd when she was rear ended by the operator of Ma Reg 6961RC being operated by Jenna Limone. Jenna stated she thought Marlow was going and accidently rear ended her. Both operators signed patient refusals with Fallon Ambulance, minor damage to both vehicles, no tows required.

On Wednesday 6/1/2022 at approx 0823hrs, while assigned to N496, I responded to the intersection of Hammond St and Middlesex Rd for a 2 car MVA. Operator of MV1 states that prior to the accident she was at a stop on Hammond St NB with a car also stopped in front of her. She said that she was struck from behind by MV2. She also said she had neck pain resulting from the collision and was transported by medics to Newton Wellesley Hospital. Operator of MV2 states that prior to the accident he was on Hammond St NB with MV1 in front him. He said that he came to a complete stop. He said then he took his foot off the break and looked behind him for a moment. When he looked back he collided with MV1. He states that he did not press his gas pedal but perhaps stopped too close to MV1. I observed no fresh damage to either MV1 or MV2.

At approximately 1810HRs on Thursday, June 2, 2022, I responded to 141 Needham Street with The Newton Fire Department for a two car crash with minor injuries. Upon arrival, I observed both vehicles in front of the aforementioned address. MV1 (MA Pass: 1HPG40) had damage to its rear end (lift gate, bumper cover, marker lights and license plate). MV2 had damage to its front end (hood, headlights, radiator core support, bumper, fenders and windshield), both air bags were deployed and it was leaking fluid. Upon speaking to the operator of MV1, they stated they were traveling straight ahead when they slowed their vehicle down and MV2 drove into the rear of it. Upon speaking to the operator of MV2, they stated MV1 decelerated and they did not have time to stop, causing them to drive into the rear of MV1. All parties involved signed patient refusals. NFD cleaned up the leaked fluid. Tody's Services removed MV2 from the roadway and MV1 drove away under its own power. I completed and filled an NPD Towed Motor Vehicle Form.

MV 1 was traveling south bound on Walnut St in front of 331 Walnut when she followed MV 2 too closely and rear ended her. MV 1 sustained major damage to its front end and was towed by Tody's for it being disabled. The operator of MV 1, Ms. Perla Heredia, was cited for driving without a license, unregistered motor vehicle, and attaching incorrect plates on the vehicle (MA reg 2KYY53). A police incident report was also completed ( incident number 22019531) MV 2 was traveling straight southbound on Walnut St when she was rear ended. MV 2 sustained minor damage to her rear end. MV 2 was able to be driven away by the operator. MV 2 was advised of this crash report number. There were no injuries to either party.

At approximately 1758HRs on Wednesday, June 08, 2022, I responded to the area of Glenn Ave. and Beacon St., for the report of a motor vehicle crash with injuries. Upon speaking with the operator of MV1, they stated they were driving westbound on Beacon St., when they slowed down and signaled their intent to turn left (southbound) onto Glenn Ave. It was at this time, they were struck in the rear by MV2. Upon speaking with the operator of MV2, they stated that while they drove westbound on Beacon St., the operator of MV1 stopped short while signaling a left hand turn. They then struck MV1 in the rear, as they did not have time to stop. MV3 then struck them in the rear. Upon speaking to the operator of MV3, they stated were traveling westbound on Beacon St. and that everything happened so fast, that they were not sure what happened, but that they drove into the rear of MV2. I noted damage to the right rear bumper cover of MV1 (MA Pass:4NAZ81) and that due to the force of being struck, their spare tire from underneath the vehicle became dislodged; an NFD firefighter placed it in their trunk. I noted extensive damage to the front and rear of MV2 (CT Pass:275RNP) and that it was leaking fluids, which NFD cleaned from the roadway. MV2's front bumper cover, grill, headlights, fenders and hood were damaged. MV2's rear bumper cover/foam was completely dislodged, its muffler was dragging and its trunk, quarter panels and tail lights were damaged. I noted damage to the grill, headlights and steel bumper of MV3 (MA Con: S15127). The operator of MV2 was transported to the hospital with injuries to their face, neck and left knee and MV2 was removed from the roadway by Tody's Services; their airbag did not deploy. I completed and filed an NPD Towed Motor Vehicle Report Form.

On 06/18/2022, while assigned to N494, I, Officer Conary, and Officer Sohn (N492), responded to an MVA at Centre Street at Garden Street. Upon arrival, I met with Operator of MV1 who explained to me that they were traveling Northbound on Centre Street, when a truck in front of them began to slow down in traffic. Operator of MV1 said that they began to slow down too and then was struck in the rear by MV2. Operator of MV1's dog, that was in the back seat, jumped out of the car and ran down the street. Operator of MV1 was able to get the dog and claimed that it was hit by the car. I observed the dog and it was energetic and was moving around without visible issues. I spoke to Operator of MV2 who explained to me that they were traveling Northbound on Centre Street when MV1 stopped in front of them. Operator of MV2 was unable to stop in time. They did not know whether or not they hit a dog during the accident. MV1 was unregistered and non renewable. Operator of MV1 was handed MA Citation T2014292. Both vehicles were towed from scene. There is damage to MV1 rear, and MV2 front end. A MV inventory form was completed for MV1. Both operators signed patient refusals. I advised Operator of MV1 to bring their dog to the vet.
