Narrative:
On 01/04/19 I responded outside of #54 Needham St for a two vehicle crash. Veh #1 was a delivery box van who was driving due North on Needham St. The operator of veh #1 stated he was attempting to take a left into #55 Needham St. when veh #2 came from behind him on his left and he struck veh #2 as he turned. The operator of veh # 1 was taken by Ambulance #1 to Newton-Wellesley Hospital for an evaluation after complaining of pain in his chest area. The passenger of veh #1 stated everything happened "so fast" he didn't see what transpired. Moderate damage to veh #1. The operator of veh #2 stated he was traveling due North on Needham St. and was behind veh #1 for a long stretch of the road. The operator of veh # 2 stated veh #1 put his right directional on three different times but didn't turn. He stated veh #1 had his right directional on and was in the right lane when he went to pass him. Veh #1 then turned into his rear passenger side as he passed. Moderate damage to veh # 2. The operator of veh #2 stated the operator of veh #1 was walking around and yelling about the accident for a period of time and then eventually grabbed his chest and fell to the ground complaining of pain. From the result of my investigation it appears as if veh # 1 attempted to turn from the wrong lane and veh # 2 was passing in the "suicide lane", also wrong lane. There will be no citations issued at this time as both operators were given verbal warnings.

MV1 pulled over to the side of the road to exchange information as MV2 fled the scene eastbound on Beacon Street. The operator of MV1 stated that MV2 appeared to be an older model sedan, possible maroon or red in color, and was operated by a white female in her 60's. MV1 sustained significant front end and passengers side damage. MV2 likely sustained significant front drivers side damage. I was unable to check the surrounding businesses for possible security footage of the crash as all of the surrounding stores were closed when I arrived on scene.

Operator of MV#1 stated he was travelling westbound on Commonwealth Ave with his dog in the front seat when he slid on ice and off the roadway. The roadway was wet from previous snow fall but was very well plowed and sanded. MV#1 hit a curb, drove through a snowbank, hit a tree with the left front of his vehicle and then hit a pole with the right front side of his vehicle. The car then came to a rest in the median. The operator appeared confused by the accident and did not realize the extent of the damage to his car.He asked what he had hit. He continuously asked if it was driveable even though the entire front end was totaled. He was able to provide me with his license and all pertinent information but spoke in a slow manner. There was no odor of alcohol. Operator declined medical attention. When asked if he was on any medication he stated he was on heart medication. We were able to contact the operator's son who was transported to the scene and stated the operator had previously suffered a stroke. David Donahue arrived on scene towed the vehicle to Tody's. A licensed driver picked up the operator. A search of the immediate area yielded negative results for ice in the roadway. A request for Immediate Threat License Suspension/Revocation was filed.

I responded to the area 107 Dartmouth St. at approx. 0345 on 04/28/9019 along with N493 Officer Kelly Donahue and N508 Sgt. Devine for a report of an MVA. The MVA was reported to be single vehicle crash. The vehicle left the roadway and was on the front lawn of 107 Dartmouth St. I arrived on location (first on scene ). I observed tire tread marks across the front lawn of 107 Dartmouth St. I then observed a vehicle in the bushes toward the left front side of the house. The vehicle; NY Reg. JHE8252, Porsche color blue. The RMV info on the plate came back registered to Porsche;of ;South. Registration plate type: Dealer Assignment. 185 W Sunrise HWY; Freeport,NY (no further info). The vehicle was unoccupied and secured. I felt the hood of the car which was still warm from the engine. I checked the immediate area surrounding the crash scene for an operator but found no one. I radioed to other responding units to canvas the neighborhood and surrounding area for the operator on foot. I spoke to the homeowners at 107 Dartmouth St., identified as Anthony and June Limberis. Mrs. Limberis stated she heard a load noise and a car engine revving. She stated when she looked out the window she observed a W/M tall, thin, wearing tan khakis and a light colored plaid shirt walking across the front lawn. He proceeded up Dartmouth St. (northbound) toward Commonwealth Ave. Newton Fire Dept arrived on location to check the crash scene and vehicle for possible leaking fluids and any other hazards. As I was talking with the firefighters we were approached by two men. One of the men was intoxicated. He was unsteady on his feet, had glassy bloodshot eyes, and was slurring his speech. The man was a W/M tall/thin approx. 6'2" and was wearing tan khakis and a light colored plaid shirt. He was identified as Eric Adams DOB 09/18/1969. The second man with him was identified as John Adams DOB 02/07/1936; An elderly W/M approx. 5'6" also wearing tan khakis and a light colored plaid shirt. Eric Adams stated his father John was the operator of the vehicle. Eric Adams had a strong odor of Alcoholic beverage emanating from his person and breath. He continued to slur his speech and was unsteady on his feet as he spoke. He was swaying back and forth and side to side as he spoke to me. Officer K. Donahue and Sgt. Devine were on scene at this time and also made the same observations. Multiple times we had to grab hold of Eric Adams to keep him from losing balance and falling. We spoke with Eric and John Adams for several minutes as their accounts of what took place were not consistent. I asked Eric Adams where the keys to the vehicle were. Eric Adams immediately pulled the starter key to the vehicle from his front right pocket. Eric Adams became defensive and belligerent when questioned about who was operating the vehicle. John Adams appeared tired (as if he just woke up) and confused as to the situation. Sgt Devine and I both spoke with Eric Adams and John Adams. Although both men claim John Adams was the operator of the vehicle, a clear determination was uncertain. A query of both mens driver's license status on NY Licenses came back active. Eric Adams has two prior DUI's on his RMV record. Officer Donahue and Sgt. Devine escorted the two men to 93 Dartmouth St. Eric Adams stated they were staying at that residence which is his sister's house. I stayed at the crash scene to document information and assist with clearing the MV from the property with Tody's Towing Service. As they were walking away I observed Officer Donahue and Sgt. Devine continue to have to assist Eric Adams with staying on his feet. Approximately ten minutes later as I was speaking with the aforementioned homeowners Mr.&Mrs. Limberis. Officer Donahue and Sgt. Devine returned to the crash scene. As they were updating me we observed Eric Adams returning to the crash scene. He was stumbling and swaying as he walked and repeatedly almost fell. Officer Donahue and Sgt. Devine immediately intercepted and assisted him before he fell. As they were assisting him again I continued speaking with the homeowners. Suddenly I heard Officer Donahue and Sgt. Devine struggling with Eric Adams who was now on the ground and resisting being handcuffed. I ran to assist them. Eric Adams continued to resist and be uncooperative until he was appropriately restrained in handcuffs. Eric Adams was placed under arrest by officer Donahue for Disorderly Conduct and Resisting Arrest. Reference Officer Donahues Newton Police Incident Report #19016484. The vehicle NY Reg. JHE8252 was removed from the property of 107 Dartmouth St. and towed from the crash scene by Tody's Towing Service.

Narrative:
On August 1, 2019 at approximately 17:34 hours I, Officer Guarino, responded with Newton Fire and Medics for a report of a single vehicle crash into the building of 335 Boylston St. Upon arrival, the operator and sole occupant of the vehicle was out of the vehicle and had minor injuries. MV1 struck a stop sign and was up against 335 Boylston St, which appeared to have a broken window and minor structural damage. Operator of MV1 said that he was traveling in the right lane of Boylston St (Westbound) when a white Mercedes sedan (unknown model or registration number) turned into his lane and almost struck his vehicle. Operator of MV1 swerved to the right to avoid a collision and as a result crashed into the stop sign on John St. and the side of 335 Boylston St. Witness 1, who was parked at 325 Boylston St. saw the incident and corroborated MV1's story. A second witness, who I was unable to make contact with stopped and gave MV1 his phone number of (508)215-4951 and texted MV1 that he caught up with the possible vehicle, MA Reg 9BT373, which comes back to DESOUZA PLASTERING SERVICES INC. I will attempt to make contact with an operator during their business hours. The owner of 335 Boylston St, Onir Spiegel, was on scene, was given the incident number and was advised to contact her insurance company. MV1 was towed from the scene by Tody's and the operator signed a patient refusal from the Medics. Pictures were taken at the scene of the damage and submitted to IT to be attached to this report. Sergeant McLean notified the DPW for the stop sign and glass in the roadway. Sergeant McLean also left a message for Newton Inspectional Services to come inspect damage to the building at a later time.

Mv#1 operator states she was pulling out forward W/B from a parallel marked parking space in front of 843 Beacon St. #1 states she was about half way into the travel lane coming out of her turn to proceed straight ahead. #1 states at this time, she was struck on her driver's side front end that took off the bumper by Mv#2 that was travelling past her at a high rate of speed from behind straight ahead on Beacon St W/B. #1 operator further states #2 had crossed over the double yellow line to drive around her as she was out into the travel lane. Mv#2 operator states she was travelling on Beacon St W/B and was struck on her passenger side by #1 as #1 was pulling out of the parking space. I observed the initial contact damage on #2 on its passenger side rear door running down to the front of the passenger side wheel well and passenger side rear tire (tire wall partly sliced from impact). When #2 operator was asked if she crossed over the double yellow line, #2 replied only after the impact. #2 added prior to the collision, she briefly looked at her GPS. After statements were received from both operators and at the conclusion of this investigation, a witness identified herself on scene. The witness stated she was sitting in her driver's seat in her parked Mv directly in front of #1. Witness stated she observed #1 pull out from behind her into the travel lane. Witness stated #1's front end had almost fully reached her driver's side door before #2 made impact. Based on statements and witness statements and it appears #2 operator did not see #1 enter the roadway until the last second before impact by overcorrecting and entering the opposite lane of travel.

The medics looked at Rizza and he was alert and had a minor laceration to his head. Rizza's mother Domenica Rizza picked him up and brought him to NWH. I drove around the area and could not find a black Honda sedan with damage to the front of it. As of now, I do not have anymore information. This report is a hit and run. Traffic Update (Officer Gaudet): Myself and Officer Anderson have both canvassed the area of the crash for identifying physical evidence from the vehicle that fled the scene with a negative result. We have also reached out to local business's in an attempt to secure surveillance footage of the crash with a negative result. Narrative:
MV#1 was traveling North on Fair Oaks Ave, and attempted to make a right turn onto California St. MV#2 was parked in front of 589 California St facing West. MV#1 appeared to have run over the curb at the intersection, hit a city tree in front of 55 Fair Oaks Ave (California St side), and then crashed into MV#2. When I arrived on scene I observed a male party with blood on his head, and another male party standing next to him, later identified as Elmer Lopez.. I asked Elmer which car he was driving, and he stated that he was in MV#1. I asked Elmer what happened, and he stated that he and the other man with him, identified as Ricardo Deras, were in the rear seat with another male party, and that there were two other parties in the front seat, one of them was driving, he was not able to provide me with a name of who was driving. I asked him where everyone else was, and Elmer stated that after the crash, the got out of the car and ran away. At this point, medics arrived on scene and began to tend to Ricardo. I next spoke with the owner of MV#2, Jim Silva. Jim stated that he was in his home when he heard a loud crash. Jim stated that he came out of the house in time to witness the two males I was speaking with looking at the car, and he also stated that he observed three parties walk away from the scene, two heading east on California St, and one heading north on Fair Oaks Ave. MV#1 and MV#2 were towed off scene by Todys. Due to the scene, and the location of the cars in a busy street, I was not able to do a MV inventory of MV#1 until after it was towed by Todys. The inventory was done at Todys Service station, and a MV inventory sheet was filled out and placed the in appropriate box. I took three pictures of the damaged tree and submitted them to IT to be attached to this report. While I was taking the pictures of the tree, two males were walking past the scene. I stopped both parties and questioned them about the incident. They stated that they had been in the car, but had not been driving. I asked them if the knew who the driver was, and they told me that they did not know the name of the driver. Due to no witnesses observing the driver and all parties stating that it was someone else, I was not able to determine a driver of the vehicle. Units checked the area for the fifth party in the vehicle with negative results. Ricardo was transported to NWH hospital by medics. All other parties were sent on their way. All units cleared without further incident. **UPDATE** On Nov 25, 2019 at approximately 1621 hrs, I contacted Westborough Police Department in an attempt to get a hold of Dora. Westborough informed me that they made an attempt to contact her with negative results. Westborough PD left my contact information for her to call me back.

On 11/26/19 at 14:11 hours, I responded to Newton South High School located at 140 Brandeis Road for a two car motor vehicle accident involving a school transport van for the City of Newton and another vehicle. No injuries were reported. The road conditions at the time were dry and the weather was a mix of sun and clouds. Traffic conditions were heavy due to the release of school at the time. Operator of MV# 1 states he was operating straight ahead eastbound on Brandeis Road awaiting the backed up traffic in front of him that was entering the right roundabout for the main entrance to the high school. While doing so, he states that MV # 2 came up and passed him on the left to go around the traffic. MV # 2 then clipped his vehicle on the driver's side front quarter panel while passing him. I observed no visible damage to the quarter panel area and scuff marks to the left front rim which possibly were from the accident. Operator of MV # 2 states she was also operating straight ahead eastbound on Brandeis Road awaiting the backed up traffic in front. She states that once there was an opening in the roadway ahead of her, she signaled her left directional and started to pass traffic on the left. While doing so, she says that MV # 1 suddenly pulled out to the left without signaling and struck her vehicle on the front passenger side. I observed minor scuff marks to the front passenger quarter panel area and slight damage to the metal grate footstep on the passenger side. The operator was also transporting two students for the City of Newton through JSC Transportation Services at the time. Both students attend Newton South High School. Digital photos taken of the scene and damage.

The owner of MV#1 stated her friend "Rosemary" who was sitting in the front passenger seat of MV#1, observed the vehicle parked next to it back out of its parking spot and struck MV#1. I spoke to the manager of Holly Cleaners and she was able to provide me with the customer's contact information that drove MV#2. The manager of Holly Cleaners was able to show me video footages of the accident. The video shows that operator of MV#2 attempted to back out of his parking spot making a sharp turn and leaving the area. The video then shows the passenger of MV#1 get out of the vehicle and looked at the damages. I am unable to determined if contact was made between the two vehicles but it was highly possible that contact was made. MV#2 appeared to be a gold colored "Audi" SUV operated by a Fred Ellis of 36 Sallbury Rd in Brookline MA (617 731-3358). I am unable to view a registration or locate a registered vehicle to a Fred Ellis at this time. The damage to MV#1 is to its driver's side rear door. It displayed a light color mark (Minor damages). There were no reported injuries to all parties involved. The front passenger of MV#1 was only known as "Rosemary" (Unknown last name, address or D.O.B). I contacted the operator of MV#2 and left a voicemail. To be further investigated pending statements from the operator of MV#2. On 01/17/2020 at 09:45 I spoke to Alfred Ellis and he stated that he was at the Holly Cleaners parking lot and could not reverse due to MV#1 parking so close to his vehicle. Mr. Ellis stated he unsure if he had made contact with MV#1. Mr. Ellis was forthcoming with his statements and provided me with his vehicle and license information. He stated he was not injured and I provided him with this accident report number along with information of the other involved vehicle.

Vehicle #1 was traveling south on Crafts St. Vehicle #1 came to a stop to take a left turn into the operators driveway in front of 297 Crafts St. when it was struck in its rear end by Vehicle #2. Vehicle #2 was traveling south on Crafts St. and failed to slow for the Vehicle #1 that was stopped in traffic. Vehicle #2 crashed into vehicle #1 then continued south on Crafts St. before turning left onto Linwood Ave and leaving the scene. While on a routine traffic stop at California and Bridge St. Officer Spinney and Sergeant Mcneil heard me give out the description of vehicle #2. Officer Spinney and Sgt. Mcneil saw a damaged vehicle matching the description pass them going east on California St. Sgt. Macneil stated the truck said "Franchi" on the side. When Officer Spinney cleared the MV stop he was able to locate vehicle #2 located behind 37 Riverdale Ave. The Trucks hood and engine was still warm to the touch and had corresponding damage that matched witness statements. I was able to match pieces of the truck left at the scene of the accident to the vehicle Officer Spinney located. Pictures of the scene and both vehicles were taken and turned into the IT bureau. The co-owner of Vehicle #2 Robert Franchi could not tell us who was operating the vehicle. The co-owner of Vehicle #2 Robert appeared to be intoxicated and admitted to drinking. The owner of vehicle #2 has video footage of the vehicle being parked but could not provide us with a copy of it. Vehicle #1 suffered minor damage to its rear but was disabled and needed to be towed from the scene. Todys towed the vehicle. A towed MV report was completed and filed. Vehicle #2 suffered heavy front end damage primarily to its driver side. No injuries were reported.

Inga Sipicina stated that at 1050 hrs a brown UPS truck bearing S79977 struck a sign that was attached to the building above the entrance in two different locations. Inga stated the truck then took off. The sign had clear but minimal damage. A branch manager from UPS later came to the store to speak with Inga and her husband Slava and stated they would be in contact with them but didn't leave any information. I attempted to call the business but it was after hours and left a message. There were no injuries involved in this accident. On 2/4/20 I again attempted to contact UPS with negative results.

Narrative:
On February 10, 2020 at approximately 1911 hours, while on patrol in Newton Centre Sq, I was approached by a woman who wished to report a past MV hit and run. Ms. Eileen Kavall stated last Thursday(02/06) between the hours of 1745 and 1830, her MV was parked in a handicap spot in front of the Star Market at 1 Boylston St. While parking her vehicle, she noticed a large white SUV parked in the spot directly next to her. She stated this vehicle was also in a handicap parking space but did not see a handicap sticker on the vehicle. When Eileen exited the store, she observed damage to her front left fender and the SUV was gone. I also observed this damage. Eileen did not have a number plate for the MV that hit her but stated the Star Market may have cameras facing where the incident occurred. I then provided Eileen with an accident report number and went to Star market to speak with the manager. At Star Market, I spoke with Janet Yaksic, store manager, who stated she remembered speaking with Eileen about the accident. Janet stated that she would have to contact the store's director in order to contact the loss prevention employees who do not work on site. The loss prevention employees would be able to review any possible footage by the end of the week and contact Newton Police with their findings. I left my name and contact info with Janet. This accident is still under investigation pending a review of the Star Market footage. Update: 02/22/20 I have not received any video surveillance from Star Market. RP will be advised to pursue with their insurance company.

Narrative:
The operator of vehicle 1 stated she was traveling West on the Washington Street bridge over the Mass Pike when vehicle 2, a gray SUV attempted to change lanes, as this happened the front passenger side quarter panel of vehicle 2 came in contact with the rear drivers side quarter panel of vehicle 1. This caused minor scratches to vehicle 1. The operator of vehicle 1 attempted to pull over and exchange information with the other driver. The operator of vehicle 2 continued driving away and got onto the Mass Pike, during this process the operator of vehicle 1 and the passenger of vehicle 1 were able to get the license plate number of vehicle 2 which was NY REG:T797800C. In an attempt to locate and identify the driver of vehicle 2 I ran NY REG T797800C which comes back registered to a short term rental company called Flexdrive. I called a phone number on their website 8663539399 which is listed as the number to call if you are in an accident with one of their vehicles. I was only able to speak to a service technician, a Tom Louis of ARI Fleet Service 18002272273, who told me the vehicle was out of the Flexdrive North Dighton Mass hub located at 123 Williams Street North Dighton MA 02764.Tom also mentioned the location would most likely not be open again until Monday. He did not have any other information regarding who may be operating the vehicle. I have attempted to find contact information on this North Dighton location which I have had negative results so far. I have also emailed the Flexdrive customer service department. To be followed up on. Traffic Bureau update (Officer Gaudet): On Tuesday, February 25, 2020, I made contact with the registered owner of MV2, Flexdrive. The representative from Flexdrive stated MV2 is registered to the company but Lyft uses their vehicles for ride sharing. I contacted Lyft and spoke with a party named Jonas from their support team. Jonas stated he would make contact with the operator of MV2 and ask them to contact me to give a statement. Lyft stated they would also conduct their own internal investigation. On Wednesday, February 26, 2020, I spoke with the operator of MV2, Gary Procaccini (S36066201). Procaccini stated he was operating a 2019 Santa Fe (NY Reg: T797800C) as a Lyft driver on the evening of February 22, 2020. Procaccini stated he was traveling on Washington Street (W) towards the Massachusetts Turnpike entrance in West Newton. Procaccini stated he was in the wrong lane and looked in his mirror before attempting to merge into the correct lane. Procaccini stated to the right of his vehicle he observed a vehicle "flying" towards the entrance to the Pike. Procaccini stated he came to stop to allow the vehicle to pass. Procaccini stated the vehicle passed at a high rate of speed and he continued on his way onto the Pike (E). Procaccini stated he had a passenger in the vehicle using Lyft servies and there were no signs that a crash had taken place. Procaccini's information has been added to his report to reflect him being the operator of MV2. I then spoke with Peter Houser who is a Driver Services Representative with Lyft. Houser stated he had just visually inspected the vehicle alleged to have been involved in the crash. Houser stated the vehicle was in "pristine" condition and he observed no damage to the front passenger side quarter panel of MV2. Houser e-mailed me pictures of the vehicle. I observed no damage to MV2. Those pictures were submitted to the IT Bureau.

On 07/28/2020 at 1221hrs I responded to the area of 23 Oak St for a motor vehicle crash involving a pedestrian. The involved pedestrian had left the area prior to my arrival. The vehicle that was involved in crash was also not on scene. The crash occurred on 07/28/2020 at approximately 0900hrs in the driveway of 27 Oak St. Prior to the crash the involved vehicle described as a 1991 Chrysler TC Convertible color Red bearing MA Antique Reg D2935 ( Active) was located in the driveway of 23 Oak St. The vehicle was facing toward the rear of the driveway. The vehicle then rolled in reverse easterly across the common driveway and struck a pedestrian who was located in the driveway of 27 Oak St. After contact with the pedestrian the vehicle came to final rest in the driveway of 27 Oak St. As a result of the impact the pedestrian sustained a minor injury to her left knee. There were no occupants in the involved vehicle at the time of the crash. It is unknown at this time if the vehicle sustained any damage. The pedestrian left the area prior to this crash being reported to NPD. The vehicle was removed from the area prior to the crash being reported to NPD. I then spoke to the owner of the involved vehicle Marc Dumoulin who was on scene and was the reporting person. Dumoulin stated that the crash occurred today 07/28/2020 at approximately 0900hrs. Dumoulin states that the involved vehicle described as a 1991 Chrysler Convertible was having issues due to a gas tank problem and he believes it wasn't going to pass inspection. Dumoulin stated that he took his vehicle for a final test ride around the block to decide if he wanted to keep it or not. Dumoulin states that after operating the vehicle he decided to salvage the vehicle with CJ Auto in Watertown, MA. Dumoulin states that before going to CJ Auto he decided to stop back at his home first. Dumoulin states he then parked the vehicle at this driveway and got out to go in the house. Dumoulin states that he believes the vehicle was running at this time. Dumoulin states that as he was walking toward his house he observed his vehicle rolling in reverse towards his neighbor's driveway. Dumoulin states that the vehicle then rolled into his neighbor's driveway near his neighbors fence. Dumoulin states that the pedestrian was outside near the fence when his vehicle rolled into his neighbor's driveway.Dumoulin states that he did not initially report the crash because he did not believe that his vehicle had come in contact with the pedestrian. Dumoulin states that he then removed the vehicle from his neighbor's driveway and brought it to CJ Auto for salvage. Dumoulin states that when he returned to his residence he became aware that the pedestrian may have been struck by his vehicle and had a minor injury. Dumoulin states that upon hearing this he called NPD. At this time I was met by Nina Levin who is the homeowner of 27 Oak St. Levin stated that she did not observe the crash but a contractor working on her house did. Levin stated that the pedestrian involved was named Lucero Flores-Roman and that Flores-Roman was outside attending to some potted plants when she was hit by the involved vehicle. Levin stated that Flores-Roman received an injury to her left knee. Levin stated that she took photo's after the crash of the involved vehicle and of Flores-Roman's knee. Levin provided me with these photo's which I then submitted to the IT Bureau. Levin provided me with Flores-Romans contact phone number. Dumoulin also provided his insurance information to Levin so that she could relay the information to Flores-Roman. I then spoke with a witness Gleb Andreev who was a carpenter working at 27 Oak St. Andreev states that he was inside of 27 Oak St working on a first floor window facing the driveway. Andreev state he observed the involved vehicle roll into the driveway and come to rest near the pedestrian. Gleb stated he then went out side to assist the pedestrian. Gleb stated he believed the vehicle hit the pedestrian when it rolled into the driveway. At 0230 hrs I was able to speak with the involved pedestrian Lucero Flores-Roman. Flores-Roman states that around 0900hrs today she was in the driveway of 27 Oak St attending to some plants near a wooden fence. Flores-Roman stated that her back was turned towards the area of 25 Oak St, she then on a hunch turned around and observed the involved vehicle in reverse moving towards her. Flores-Roman stated that she turned her body quickly to avoid the vehicle but the vehicle's rear end passengers side made contact with her left knee. Flores-Roman stated that she sustained a minor injury to her left knee which included some bruising and swelling. Flores-Roman states that after crash she did not think she needed an ambulance but wanted to go home to rest her knee. Flores-Roman was then provided with Dumoulin's insurance information. No other injuries reported on scene. Photo's were taken on scene and were submitted to the IT Bureau. On 07/29/2020 I responded to CJ Auto's Salvage Yard located at 43 White Pond Rd Stow, MA which was the present location of the involved vehicle. Upon arrival I observed the involved vehicle which did not appear to have exterior damage. Photo's were taken of the vehicle and were submitted to the IT Bureau. An RMV check of vehicle revealed that a title was issued to Dumoulin on 03/25/2020. The vehicle has not been inspected by a MA inspection station. The vehicle title has been signed over by Dumoulin to CJ Auto for destruction. At this time it is unknown if the crash was caused by mechanical failure with the vehicle or human error.

Narrative:
On August 1, 2020 I responded to a single motor vehicle crash at 1200 Walnut St. I spoke to the owner of MV1 who said that he parked MV1 in front of 1177 Walnut St to run across the street and pick up his mail. When he exited his vehicle, he noticed that his vehicle began rolling southbound on Walnut St and crashed into city poles in front of 1200 Walnut St. The owner of MV1 said that he had recently got his vehicle inspected but periodically would have the brake light come on and was planning on contacting his mechanic. MV1 has a manual transmission, and the owner said that he put the vehicle in gear, shut the vehicle off and used the electronic emergency brake. He believes that the emergency brake may have malfunctioned, causing the vehicle to roll southbound on Walnut St and crash. As a result of the crash, MV1 sustained front end damage and there was visible damage to the city poles (used to protect the tree on the sidewalk). Officer Raymond took photographs of the damaged city property and submitted them to IT to be attached to this report.

On 09/09/20 I met with William Wrang the property manager of 85 Wells Ave. Wrang states that on 09/08/20 between 1200 and 1500 hours a delivery driver for A. Duie Pyle backed into a brick wall near the loading dock causing a good amount of damage to the wall. Wrang was notified by a tenant of the building who stated the delivery driver made no mention of the damage but seemed to be in a hurry to leave. The damage was noticed by the tenant after the driver had left. Wrang has already reached out to the delivery company and gave them the tracking number of the package (478769557). The company stated they would pass along the information to their claim department and someone would be in touch. Wrang has still not heard back and wanted the incident documented. There are no cameras in the area. Traffic Bureau update (Officer Gaudet): On Thursday, September 17, 2020, I contacted Duie Pyle Inc and spoke Mr. Tim Hurford. Mr. Hurford stated the incident was not a Hit and Run and that his driver contacted the company and filed a report on the same day as the crash. Mr. Hurford stated the company has already received an estimate for the damage caused to the building and are working with the property owner to resolve the issue. Mr. Hurford stated the operator of the vehicle was identified as Mr. Joeli Perez-Marte (S97970226). Mr. Perez-Marte has an active license. No further action will be required.

On Tuesday October 13, 2020 at approx 1323 hours I went to the area of Anthony Rd at Anthony Cir for a report of a past hit/run that occurred at approx. 1215 hours in the westbound lane of 697 Washington Street. Upon arrival I met the operator of v1 Ms. OLGA KHOLODENKO, she stated she was traveling westbound in the left lane of Washington Street around the Whole foods area which is 697 Washington Street. She stated while traveling a red truck with a red trailer was on her right side traveling along side her, and while traveling the trailer had "swung" into her passenger side of her vehicle. She stated the truck/trailer kept traveling and she followed the trailer and took pictures while driving. She then went home and called her insurance company with the picture of the registration plate and they gave her the information which brought them at Anthony Rd at Anthony Cir, where the registered owner lived. The truck / trailer was not at the location. The trailer is MA trailer reg # C39390 a 2013 Brav trailer color red registered to CIRO CARBONE AND SONS INC 12 ANTHONY CIR NEWTON MA. Her vehicle is a 2020 Kia utility color white MA reg. 6ZG351. I observed some red transferred paint on her passenger side from the rear door to the front door. I advised them of process and I would look into the trailer. I was able to find the truck/trailer on California St at Dalby Street. I looked around the trailer and observed a minor scratch on the driver side of the trailer near the front of the trailer. I contacted the owner who due to the heavy rain all day sent home his employees. I will update oper 2 statement when I talk to him. At this time it appears with the trailer scratch being minor a reasonable person would have not known. Update: Monday October 19, 2020 I spoke with the oper of v2 via phone, he stated he was on Washington Street near Whole Foods traveling westbound with the truck/trailer. He stated he was traveling in the right travel lane. He stated the truck/ trailer was always in the proper slow lane and didn't know about the contact until I notified him. I advised him of the crash process.

The owner of MV#1 stated she came out of work at NWH at 0730hrs and noticed the passenger front of her vehicle was damaged. She stated that she parked her vehicle in the that space at 2330hrs the night before. I observed significant damage to the passenger front of the vehicle. It should be noted that I did not observe and parts on the ground or any debris indicating that the accident occurred in that location. The headlight was totally shattered out and there were missing parts from vehicle. There was also a fluid trail that appeared to be leaking from the front passenger side. This trail indicated that the vehicle was driven and backed in to the parking space after the collision took place. Based on my training and experience I do not believe that vehicle was struck in that location as a hit and run. There were no cameras in the area. The owner requested her own tow for the vehicle. I submitted a photo of the claimed hit and run scene to the IT bureau and it was attached to this accident report.

I observed damage to the rear left side bumper, a cut in the bumper plastic revealing the metal bumper frame underneath it and black friction mark on the surface of the bumper. -There were no injuries or other property damage reported at the time of this interview. This report will be forwarded to the Traffic Bureau for investigation. Ms. Nieman stated she was unaware if the property had cameras but has sent and request to the building managers. I contacted the building manager Ms. Madison Hebb. Ms. Hebb stated she is not sure if the area of the building where the alleged crash took place has cameras, but she is looking into it and will contact me when she finds out. I contacted the Newton Fire Department to try and speak with Lt. Foli who was the incident commander for the call at 2 Wells Avenue (CAD 21012488). Lt. Foli is not back to duty until this Friday. I will attempt to make contact with him during his tour of duty at Station 10 at this time. If it is determined that a Newton Fire vehicle made contact with MV1, I will update this crash report. I spoke with Ms. Danielle Subramaniam, the owner of the damaged vehicle (MV1). Ms. Subramaniam stated she did not observe a Newton Fire vehicle hit her vehicle while they responded to 2 Wells Avenue the morning her vehicle was damage. Ms. Subramaniam stated she believes the only vehicles that were in the parking lot near her vehicle during her shift were Newton Fire vehicles. Ms. Subramaniam stated when she left work after her shift that morning, she observed new damage to her vehicle. On Tuesday, March 23, 2021, Miss Hebb updated me that the side of the property where the alleged incident took place does not have any surveillance cameras present. At this time, I am unable to conclude that a Newton Fire Department vehicle caused the damage to Ms. Subramaniam's vehicle.

MV2 then backed up and left the area in an unknown direction. As a result of the crash MV1 sustained minor rear drivers side damage. MV2 likely sustained minor front passengers side damage. Monica Carney, who was sitting in the front passengers seat at the time of the crash, described MV2 as a black sedan with a partial MA Reg. of "6LT". Traffic Bureau update (Officer Gaudet): I was able to obtain footage of the crash from Chestnut Hill Security. The video captures the crash from a distance. It showed a newer model black station wagon (possibly a Subaru) enter the parking area traveling North near MV1. MV2 is observed attempting to take a left turn into a vacant parking spot next to MV1's driver side. MV2's front driver side area is observed crashing into MV1's rear driver side area. MV2 then backs away from MV1 and continues through the parking lot towards Seasons 52 and leaves the area. I was unable to obtain the vehicle's registration. Chestnut Hill Security stated they will keep checking their camera system to obtain better footage of the crash. At this time I am unable to identify the vehicle and it's operator. The footage was submitted to the IT Bureau.

Narrative:
On 4/17/21 at 0905hrs I was dispatched to 1178 Walnut St for a report of a past hit & run MVA. On arrival, I met Ms Debra Kim who had just pulled over at that address after noticing a city parking meter (zone#2461 ) was run over which was pushed onto a city tree (minor tree damage) and then the front of the building including a front door support post was struck by an unknown MV. Building damage appears minor. Building is vacant, no tenants. Ms Kim stated her father Dae Kim is the owner of the building. Crash debris left behind (no plates found) indicated the unknown MV drove off Walnut St N/B, crashed into the meters, over the sidewalk and into the building. Photos of the area were taken and will be submitted to the IT bureau. City notified. Prior to completing this report, Ms Kim called NPD to report the neighbor from 1176 Walnut has video surveillance from the crash. Ms Kim stated according to the video time, the crash occurred yesterday 4/16 at 0715hrs. She described watching on the video a black sedan possibly a Buick crash into the building back up and continue on Walnut St N/B. She stated no plates, operators or passengers could be seen on the video. Ms Kim will email me the video.

Narrative:
Vehicle 1 was legally parked facing east on the eastbound side of Webster Street. The owner of vehicle 1 states she heard a loud crash and thought someone may have struck her vehicles mirror. The owner of vehicle 1 went outside and saw substantial damage to the rear and left side of her vehicle and a left rear flat tire. Approx 10 min passed between hearing the crashing sound and calling the police. Based on the direction of debris it was determined motor vehicle 2 most likely continued east on Webster. I checked the surrounding area with negative results. Vehicle 2 would have damage to the front and right side. No identifiable debris from vehicle 2 was left at the scene. Vehicle 1 was pulled into the driveway at this address to await a tire change. UPDATE: On Monday 4/19/2021 at approx 1330hrs, while assigned to N493, the owner and operator of MV2, Ian Sotoloff, came into the Newton Police station to report that he was involved in the accident at 36 Webster St on Saturday 4/17/2021. Sotoloff states that he did not remain at the scene of the accident because he suffers from anxiety. He said he was coming from work in Natick. MV2 and operator information has been updated.

Narrative:
On May 16th, 2021 at approximately 09:42 hours I responded to the area of Centre Ave @ Washington St for a MV crash. On my arrival I located both involved MV'S on the Centre St Bridge (N/B)side. . Vehicle # 1 was a Green Toyota sedan, MA. reg. 3452-yd, operated by VEDRANA FIAMENGO. She stated she was heading N/B over the Centre St Bridge when she collided with vehicle #2 as they were both entering the same lane. Impact spun vehicle #1 around so it was facing south. Vehicle #2 was a Grey BMW MA reg. 6TR-878, operated by Thomas Brady. He stated he was merging with other traffic on the Centre St Bridge when he collided with vehicle #1. Vehicle #1 originally came from the east side of Centre Ave before coming onto the bridge. Vehicle #2 came from the west side before entering onto the bridge. The east bound traffic coming from Centre Ave has a yield sign before entering onto the Centre St bridge north bound. The traffic coming from the west side onto the bridge have the right of way. Due to the accident occurring a little further down from the merging point its unclear of which vehicle is possibly at fault. The diagram doesn't depict the actual crash scene since vehicle #2 had moved further down the street so as not to block other traffic. There were no injuries due to this accident and neither vehicle was towed.
