Operator of vehicle one stated that while driving westbound on Adella Ave the rear driver's side of his vehicle struck the front driver's side mirror of vehicle two. There was no damage to either vehicle. Vehicle two was parked unoccupied in front of 30 Adella Ave at an angle.There was another unoccupied vehicle parked across from 30 Adella Ave further constricting the roadway. Both occupants of vehicle one stated that they were not injured. Photos of both vehicles were taken and the disk was downloaded by the N PD's IT bureau. Adella Ave is a pubilc way in the City of Newton.

On 1/21/19 I was working N498 when I responded to 17 Charles River Ter for a report of a Cab that had struck a wall. Upon arrival I met with Pema Jigmey and Miriam Jost. Jost stated that Pema who was driving for Metro Cab (or Photo Cab) picked her up at the airport and brought her home. Once at her home 17 Charles River Ter, she asked him to back into the driveway due to the snow and ice conditions. While backing up in the driveway of 17 Charles River Ter the vehicle struck the residencet causing damage. Pema stated the same thing but believed Jost was half to blame due to the fact she asked him to be dropped in the driveway. I informed Pema that he drove the vehicle into the house and so was at fault. The house only appeared to suffer minor cosmetic damages but could not be accurately examined. Pema and Jost exchanged information in the hopes of settling the damages without insurance. Both parties were satisfied and provided a report number. Both parties stated they were uninjured and refused medical attention.

The operator of MV1 stated he was traveling Northbound on Centre St. at the intersection of Hyde Ave. when a vehicle in front of him braked. When he stepped on his brakes to slow down, MV1 skidded on ice off to the right into the "Speed Limit 25" sign, knocking it over. Tody's responded and pulled the vehicle off of the sign and the vehicle was able to be driven from the scene safetly. No injuries were reported. Photos of the sign were submitted to the IT bureau to be attatched to this report.

At approximately 1615HRs. on Thursday, January 24, 2019, I was dispatched to the intersection of Elliot St. and Boylston St. for a report of an MVA. Upon arrival, I located the operator of MV1 (MA Pass: 1NB459) who had pulled into the BP gas station. He stated that he was traveling Northbound on Elliot St. when his traffic signal turned red and he brought his vehicle to a complete stop at the intersection. He stated that at this time, he was rear ended by MV2. He and the operator of MV2 then got out of their respective vehicles and spoke about the collision, after a few moments, the operator of MV2 got in her vehicle and left. Using the picture of MV2's license plate (MA Pass: 3EY477) which he provided me, I was able to contact the operator of MV2 at her residence. She stated that she was traveling Northbound on Elliot St. and observed MV1 come to a stop and she applied her brakes and came to a complete stop aswell, without making any contact to MV1. She further stated that the operator of MV1 became agitated and started to yell at her. She then left because she asserts that there was no collision and she was concerned by the operator of MV1's behavior. I observed no damage to MV1's rear bumper area, with the exception of an old dent, approx. the size of a quarter, which was highlighted with some type of permanent marker. I observed no damage to MV2's front bumper area and I noted large, pronounced license plate bolts affixing MV2's license plate to its bumper. The operator of MV1 stated that he was suffering from back pain due to the collision and elected to drive himself to Carney Hospital. Due to there being no damage to either vehicle and the large plate bolts on MV2, which would have likely caused notable damage to MV1's bumper, I will not be charging the operator of MV2 with leaving the scene of an accident at this time. Both parties have been advised to contact their insurance companies and I cleared without incident.

Vehicle 1 was traveling east on Washington Street. The operator of vehicle 1 states he felt the steering wheel pull hard to the right. The operator of vehicle 1 then pulled the steering wheel to the left in attempt to correct the pulling to the right. At this time the vehicle began to pull harder to the right and ended up striking the curb and a concrete light pole. The concrete pole is owned by the City of Newton, pictures taken and submitted to IT. MED2 responded and obtained a patient refusal. Eversource was notified and responded to disconnect the power. After the power was disconnected Todys was able to remove the vehicle. I was able to move the disconnected light pole and some debris to make the sidewalk passable. Eversource placed a cone over the disconnected wires and I was able to tape off the downed pole. Dispatch notified the city for the pole and to clean up the debris.

On 2/13/19 at approximately 2045 hours, I responded to a single vehicle accident. The operator was driving northbound on Woodland rd and was taking a left hand turn on to Maple st when the car slipped on the snow/ice and collided with the no parking sign on the sidewalk. No injuries reported. Todys was notified to help assist getting the vehicle off of the sign. Two pictures were taken and submitted. The city was notified of the sign being down and in need of a replacement.

On Thursday, February 21, 2019, while assigned to Traffic unit N526, I responded to the area of Ionia Street, Newton for a report of a motor vehicle accident involving a City of Newton owned vehicle. Ionia Street is a public way maintained by the City of Newton. The weather at the time of the accident was overcast. The road surface was icy and snow covered. The operator of MV1, Patrick Carresi (MA: S73958042), stated he was operating a City of Newton owned 2016 Ford F350 plow truck (MA MVN: M94756) turning left onto Ionia Street from Auburndale Av. Carresi stated as he took the turn, the rear passenger corner of the truck made contact with the drivers side front panel of MV2 which was parked on the side of the road facing southbound. I observed minor damage to the rear of MV1. MV2 received heavy front end damage with the entire front panel torn off. Carresi reported no injuries. Carresi works for the Utilities Department with the DPW. The operator of MV2, Donna Canavan (MA: S57637058), stated she was parked inside her 2005 Honda CRV (MA: 8ZEE40) facing southbound on Ionia Street. Canavan stated as the plow truck took the turn onto Ionia Street from Auburndale Av its rear corner of the truck struck the front panel of her parked vehicle ripping the panel off. I observed significant damage to the front panel area of MV2. Canavan reported no injuries and was able to drive her vehicle from the scene. Pictures were taken of both vehicles and submitted to the IT Bureau.

Operator of vehicle one ( MA reg 3986BV 2012 MERZ ) , Vladimir Krapchev stated that on 03/21/2018 at 08:56 hours he was stopped at a red light on Center St at Commonwealth Ave. Krapchev stated that his vehicle was heading south on Center St and was the third vehicle in line to turn west bound on Commonwealth Ave. Center St and Commonwealth Ave are both public ways in the city of Newton. Krapchev stated that " a red/maroon full size sedan fire department car " struck the front driver's side fender of his vehicle. Krapchev stated that the fire department car was travailing south bound on Center St in the north bound lane, passing stopped south bound Center St traffic. Krapchev stated that there was also north bound traffic on Center St that was turning from the west bound lane of Commonwealth Ave. Krapchev stated that the fire department car turned onto the east bound lane of Commonwealth Ave and did not stop. Operator of vehicle two Newton Fire Department LT Phillip McCully stated that he was driving Newton Fire Department Car 14 ( MA reg MVN MF461 2004 Ford Crown Victoria ) south bound on Center St with his vehicle's emergency lights and siren activated. Lt McCully stated that he was responding to a motor vehicle crash with possible entrapment on Hammond Pond Parkway. Lt McCully stated that he was passing stopped Center St south bound traffic in the north bound lane of Center St. Lt McCully also stated that there was north bound traffic on Center St and " it was tight between the cars ". Lt McCully stated that he was unaware that a crash had occurred. Lt. McCully stated that he did not fell and contact between his vehicle and Krapchev's vehicle. Lt McCully also stated that he did not hear the crash because of his vehicle's siren was activated. Krapchev's vehicle had minor damage and red paint marks on it's front driver's side fender. The Fire Department car had scuff marks on it's passenger side rear door. Neither vehicle was towed and both operators stated that they were not injured in the crash. I took photos of both vehicles and forwarded the disk to the NPD It bureau for down loading.

The operator of vehicle 1 stated that with a green left turn arrow showing, and her left directional on she went to turn onto Waltham Street from Crafts Street when all of the sudden she hit a bicyclist. She stated she did not see the bicyclist at all prior to him hitting her car. The bicyclist stated does not remember anything from the accident at this time. The bicyclist was traveling southbound on Crafts Street and appears to have entered the intersection of Crafts and Waltham Street without stopping and while a red light was showing. He was wearing a helmet at the time of the crash, he was not wearing any reflective clothing(dark green jacket), and the bike did not have any lights. The bike was taken to support services for safe keeping by Ofc. J.March. Vehicle#1 was towed away by Tody's towing due to extensive damage. Bicyclist was transported to Beth Israel hospital for several trauma injuries. Pictures of scene was taken by me and sent to IT Bureau. Both witness's statements were the same. They stated that they were behind vehicle 1 in traffic and saw that she had on her left dirrectional signal and had a green left arrow signal when vehicle 1 went to turn left onto Waltham Street.

ADVISED TO CONTACT INSURANCE COMPANY. CITY OF NEWTON WAS NOTIFIED. I PLACED THREE ORANGE CONES AROUND THE HOLE TO AVOID FURTHER ACCIDENTS. COMP. On Wednesday, April 10, 2019 while assigned to Traffic unit N525, I responded to the intersection of Centre Street and Wesley Street, Newton for a report of a motor vehicle accident involving a bicycle. It was also reported that the parties involved were arguing with each other. The weather at the time of the accident was cloudy and the road surface was dry. Centre Street and Wesley Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Abdulaziz Stewart (S04075169). Stewart stated he was operating his 2017 Toyota Camry (MA: 669FM3) at the time of the accident. Stewart stated he came to a stop on Wesley Street (E) at Centre Street and was attempting to take a left turn onto Centre Street. Stewart stated there were no vehicles in the Southbound lane of Centre Street and there was a vehicle stopped in the Northbound lane of Centre Street motioning to him to take his left turn. Stewart stated as he began to start his left turn, a bicyclist crashed into the front passenger area of his vehicle. Stewart stated the bicycle had been traveling Northbound on Centre Street in the Southbound Lane. Stewart stated after the collision the bicyclist was yelling at him and called him a "Nigger". Stewart and his occupant, Miriam Stewart, reported no injuries. Miriam is a student at Bigelow Middle School. I observed minor damage to the front passenger quarter panel area of MV1. I spoke with the bicyclist involved in the accident, Timothy Murtagh (S49233464). Murtagh stated he was traveling Northbound on Centre Street on his Cannondale bicycle in the Southbound lane. Murtagh was not wearing any safety equipment while operating his bicycle (helmet) and was wearing a dark red jacket. Murtagh stated as he approached Wesley Street, MV1 "jammed on the gas" and hit him in the roadway. Murtagh stated he then fell to the ground and landed in the middle of the street. While speaking with Murtagh I told him it was reported to me by the operator of MV1 that he used a racial slur during their interaction. Murtagh acknowledged that he did and stated, "I called him a nigger". I observed moderate damage to the front wheel/rim of Murtagh's bicycle. Murtagh reported no injuries. Murtagh was advised and issued Massachusetts Uniform Citation T1443441 for Chapter 85, Section 11B (Fail to ride on the correct side of the roadway). Photos were taken at the scene and submitted to the IT Bureau. Due to the language used by Murtagh during his exchange with Stewart, Officer Rosenbaum of the Community Services Bureau was made aware of the incident for follow up.

The operator of MV#1 states that she was traveling north on Melrose St from Auburn St when she encountered a large flat bed truck blocking the roadway in front of #283 Melrose St, which is currently under construction. The operator of MV#1 states she then spoke with an individual whom she believes to be Steven Vona of Vona Construction who is currently developing the site at #283 Melrose St. According to operator #1, Mr. Vona told her she needed to turn around immediately, which she attempted to do when the front of her MV struck a raised sidewalk causing damage to the front of her MV. Operator #1 was very concerned that there was no signage or personnel from Vona Construction directing motorist where to go and feels he is responsible for the damage to her MV as he was the one informing her she needed to leave the "construction zone" immediately. She further states that no one assisted her in turning around, her view was obstructed because her MV is so low to the ground and that a construction truck was blocking the nearest driveway, making it even more difficult to maneuver. As I entered Melrose St from Comm Ave when I arrived I observed no signage and MV's were having to turn around. I informed Steven Vona that if he was going to temporarily block the street to unload equipment he needed to either use cones, signs or workers to assist drivers. In regards to the crash, Mr. Vona stated that as operator #1 attempted to turn her MV around the front of her MV struck the sidewalk. On Tuesday, April 16, 2019 while assigned to Traffic unit N525, I responded to the area of Plymouth Road and Cochituate Road, Newton for a report of a motor vehicle/pedestrian accident involving a City of Newton vehicle. The weather at the time of the accident was clear and sunny. The road surface was dry. Plymouth Road and Cochituate Road are both public ways maintained by the City of Newton. The injured pedestrian, Officer Steven Emmanuel, was transported to Newton Wellesley Hospital prior to my arrival on scene. I spoke with the operator of MV1, Officer Justin Lau. Lau stated he was on duty in his official capacity as a Newton Police Officer while operating a City of Newton owned 2018 Ford Fusion (MA: 9PYM80, Newton PD #1319). Lau stated he was parked on Plymouth Road (N) at Cochituate Road and was speaking with Officer Steven Emmanuel. Lau stated Emmanuel was standing next to the front driver side door area of his vehicle in the roadway. Lau stated a large delivery trucked approached their location on Plymouth Road from Woodward Street and was attempting to make a right turn onto Cochituate Road. Lau stated Emmanuel directed him to back his vehicle up so the delivery truck could get by their location. Lau stated he cut the wheel of the vehicle he was operating to the right; and very slowly began to back up from his location. Lau stated he was attempting to back the vehicle up to get closer to the curbing and out of the roadway. Lau stated as the vehicle was in motion the front driver side wheel made contact with Emmanuel's left foot. Emmanuel's left foot subsequently became lodged in the front driver side wheel well area. Emmanuel fell to the ground in obvious pain. Emmanuel directed Lau to pull the vehicle forward in an effort to release his foot. Emmanuel's foot was freed and NPD Dispatch was notified of the accident. Pictures were taken of MV1 and submitted to the IT Bureau. I observed no damage to the front driver tire area. I spoke with Officer Steve Emmanuel in the Newton Wellesley Hospital emergency room where he was being evaluated for a left foot injury. Emmanuel stated at the time of the accident he was working a paid detail for Paramount Pictures on Plymouth Road in his official capacity as a Newton Police Officer. Emmanuel stated he was speaking with Officer Lau on Plymouth Road at Cochituate Road. Emmanuel was standing in the street on his post and Lau was sitting in MV1 (parked). Emmanuel stated a Fed Ex delivery truck approached their location in the roadway and was attempting to get by them. Emmanuel stated Lau began to back his vehicle up in an effort to get out of the way of the delivery truck. Emmanuel stated the front driver side tire of MV1 ran over his left foot. After the tire ran over his foot, it then became lodged in the wheel well and he fell to the ground. Emmanuel stated his foot was freed from the vehicle and he was transported to Newton Wellesley Hospital by Newton Medics for further evaluation.

On Thursday 04/18/209 at approximately 1222 hours I responded to 81 Oak Street for a motor vehicle accident involving Newton Fire Ladder 2. At the time of the accident the weather was cloudy and road conditions were dry. Oak Street is a public way in the city of Newton. Upon arrival I spoke with the operator of MV1 who stated while trying to manuever through oncoming traffic on Oak Street heading southboud MV1 struck MV2's drivers side mirror while MV2 was parked. MV1 reported no damage and reported no injuries. MV2 reiterated the same events and reported very minor damage to the drivers side mirror. No injuries were reported. Several pictures were taken of both vehicles and submitted to the IT Bureau.

On Thursday 04/18/2019 at approximately 1400 hours I responded to 37 Elm Street for a single vehicle accident with City of Newton property damage. At the time of the accident the weather was cloudy and the road conditions were dry. Elm Street is a public way in the city of Newton. The operator of MV1 identified as Silton Barrett (S17437114) stated he parked his UHAUL van in front of 37 Elm Street to make a delivery. While attempting to pull MV1 back into the traffic lane he struck a fire hydrant knocking it over and completely detaching it from the base on the ground. MV1 sustained minor front end damage and was able to leave the scene. Several photographs were taken and submitted to the IT Bureau. City of Newton was notified for the property damage.

The operator of Vehicle #1, identified as Ms.Erzsebet Piros, stated that she took a left turn from Walnut St onto Washington St prior to the colliding with Ms. Kim Nguyen. She further stated that she did not see the pedestrian prior to hearing the collision. Her passenger, identified as Ms. Amy Torres, was sitting in the front passenger seat. She stated a similar account, and did not see the pedestrian prior to the collision. Neither party complained of injury, when asked. A witness, Ms. John Angelucci, was travelling in his vehicle, in the lane next to Ms. Piros. He stated that he only heard the collision, and couldn't see it due to vehicle lights, and the falling rain. The pedestrian, Ms. Kim Nguyen, stated that she attempted to cross the street, when she did not see any cars coming. She stated that she didn't see Vehicle #1 approaching until it was too late to get out of the way. It should be noted that Ms. Nguyen was not attempting to cross inside of a crosswalk, and was wearing dark colored/Navy Blue clothing. Ms. Nguyen was transported to Beth-Israel Hospital, and was communicating effectively prior to transport. Pictures of the scene, and damage to the vehicle were taken, and sent to IT for entry.

A vehicle had let Operator 1 turn onto Ober Rd and while turning a Bicycle struck the passenger side of her vehicle causing minor damage. No injuries to Operator 1 or Passenger of the vehicle. No tow needed and no citation issued. The operator of the bicycle was traveling (w/b) on Brookline st passing the cars on the right side. The bicycle did not see that there was a vehicle stopped in traffic allowing MV 1 to turn onto Ober Rd. Minor injuries to the operator of the bicycle on hands and his right knee. Medics responded and got a signed patient refusal. Pictures were taken of the vehicle, operator of the bicycle and turned into IT for attachment to the report.

On 05/10/19 I met with Luiz A. Lopes who stated his vehicle ( Mass Commercial Reg# T83695 White Toyota Tacoma P/ U) was struck while parked in the lot around 07:00. Lopes said he didn't call the Police right away because he wasn't sure his car was struck there. The truck sustained a cracked front bumper and the fog light was popped out. We went inside of National Lumber and spoke with a manager who said he would contact G.M. Greg Assarian and have him look at the security footage. On 05/14/19 at approximately 14:00 the Police Station received a call from Lopes who just got off the phone with Assarian who found some security footage of the incident. I then traveled over to National Lumber and met with Assarian. He showed me the video and it appears that a newer grey four door Chevrolet truck backed into Lopes truck around 06:58 of 05/07/19. The Massachusetts plate is unreadable but an older White heavy set male gets out of the truck and walks into the store. After parking the truck the male can be seen checking his own truck bumper for damage. The male is wearing a baseball hat and sun glasses and cannot be identified on the video. The male does however walk with a noticeable limp. Assarian will show his picture to his employees and contact me if the employees are able to identify the male driver. I called Lopes back and apprised him of my findings. ******************UPDATE**************************************** As of today 05/20/19 I have yet to hear back from anyone at National Lumber so I will end this investigation and label it a hit and run crash.

Bicyclist, Tasker Howell, was traveling SOUTH on Watertown St, entering the lane of traffic when MV1 collided with her handlebars and front tire. The operator of MV#1 stated he was travelling northbound on Walnut St when the top portion of his vehicle caught some low hanging wires and took down 2 utility poles and damaged a third utility pole. There were no reported injuries to the operator of MV#1 and no visible damages to MV#1. The operator of MV#1 is a delivery driver that is familiar with the area and frequents the same route. There were several live primary wires down on Floral St and Walnut St. Unit N-498 Officer Raymond blocked off Walnut St at Centre St, Unit N-502 Officer Nardelli blocked off eastbound Floral St and I blocked off Walnut St at Lincoln St. The surrounding pedestrian sidewalks were all taped off with Police caution tape. MV#1 struck the low hanging wires on Walnut St at the intersection of Floral St. Utility pole #A (47/12 ) located on Walnut St directly in front of the parking of the "Orchid Dry Cleaners" at 1175 Walnut St was cracked at the base. Utility pole #B was located on the westbound side of Floral St was split at the base and was dangling over the parking lot at 1175 Walnut St. Utility pole #C was located on the eastbound side of Floral St and was completely removed from its foundation and was now leaning on the building at 1182 Walnut St. The street/ stop sign of Floral St was damaged with the "Stop" sign and the "Floral St" sign completely removed from the pole (Located on the eastbound side of Floral St). A city tree limb that was damaged and landed on top of parked Vehicle #2 (Parked on the eastbound side of Floral St across from 49 Floral St. Wisconsin registration 850TGM). The owner of vehicle #2 was informed of the damage. Traffic Officer Gaudet took digital photographs of the damaged street/ stop sign and the damaged city tree. The witness Pat O'Hara of 1185 Walnut St stated MV#1 did not cause the accident but a previous bigger truck did. All he could provide was that the vehicle was a big truck travelling northbound on Walnut St. The witness Edna Jacquet stated she was parked on Walnut St (Parked between 09:00 to 09:15) at the MBTA bridge facing southbound when she observed the accident. Edna stated she observed the operator make eye contact with her and she proceeded to honk her horn in attempts to stop him. Edna stated the vehicle that caused the accident was a white colored 18 wheeler type of truck with the markings of "3 letters" all in different colors. Edna have a company video camera that faces forward attached to her vehicle that may have captured the accident. I spoke to her company manager Lisa Sanella at "TransAction Corporate Shuttles" located at 5 Wheeling Ave in Woburn MA and she will notify me if she is able to view video footages. The building owner of 1170- 1176 Walnut St Stas Gavronsky have a surveillance camera that faces Walnut St southbound towards Floral St. He will review the video footages once power have been restored that the area and notify me. On 01/07/2020 Lisa Sanella contacted me stating that they were able to review video footages from the front facing camera of Edna's shuttle bus. She stated that the involved vehicle was described as a big white colored 18 wheeler type of truck and did not have a front registration plate but did display the commercial company name of "GHG Logistics". Lisa stated the accident was time recorded at 09:13. Lisa made a copy of the video footage and will attempt to email it or deliver a flashdrive to me. An internet search of GHG Logistics showed that it is a distribution company out of 6390 Hedgewood Dr suite #300 out of Allentown PA. This company distributes wholesale sushi to the northeast area and their delivery trucks are equipped with "Rydsmart" GPS tracking devices enabling them to know their locations at all times. I spoke to Joanna Gerwig who is the store support team leader of the "Wholefoods" located at 916 Walnut St inquiring about a possible sushi delivery and she confirmed that GHG Logistics does regular deliveries and did deliver items yesterday morning between 08:00 -09:00. She stated that the truck displayed trailer #250634-481801 and that the driver was described as a big sized white male by the name of "Will" with a contact number of (570) 350-3453. She also have video footages of the truck making the delivery and provided me with their contact number of (800) 668-7700 and their corporate number of (484) 223-4502 (Confirmed to be the same company as my internet search). I viewed the video footage from Transaction Corporate Shuttles and it showed the 18 wheeler truck was travelling northbound on Walnut, catching the utility wires above and causing an electrical spark on the eastbound side of Floral St. It showed the truck slowed down and then continued to travel northbound. I spoke to Will at (570) 350-3453 and he confirmed he was making deliveries to the Wholefoods and was on Walnut St at approximately 09:00. Will stated the top corner portion of his truck did catch the above wires and heard a "twang". Will stated he briefly stopped and did not observed any damages so he continued to make his deliveries. Will stated he have been making the same delivery route for 2 years and never had an issue with low hanging wires. Will's full name is William L. Gray and have a Pennsylvania driver's license # 24551299. The truck's registration is apportioned Indiana 2917618. I have forwarded the video footage to the I.T bureau and it was attached to this report.

On 01/09/20 at approximately 13:30 hours, a pedestrian Scott Lorenz came in to the lobby of Newton Police Headquarters to report that he was struck by a motorist while crossing the intersection of Cherry & Webster Streets. This accident took place at approximately 13:00 hours on this date. Mr. Lorenz further stated that he was walking on foot and attempting to cross Cherry Street going eastbound at Webster Street. He says that he did not activate the cross walk signal but thought it was clear to proceed as Webster Street had a red light signal. While attempting to cross, he says that the westbound lane of Webster Street received a green light and MV1 proceeded to turn left on to Cherry Street from Webster Street. He then looked at the motorist and thought that she saw him. Once she kept approaching and realizing that he was not seen, he started to back track but was struck by the front end of MV1. Lorenz then tumbled back and fell down to the ground. He says that he got up feeling fine and alerted the motorist that he was struck. The two of them exchanged information and then left the scene with out notifying 911 of the accident. After, Lorenz then came in to Newton Police Headquarters to report it. I observed a small round swollen red mark to the inside of his right knee area and a red mark to the back left side of his neck. Digital photos were taken of his injuries. Mr. Lorenz declined medical attention and wanted this incident documented in case his possible injuries worsened. I then spoke to the operator of MV1, Sharon Pywell (MA S23277423), on the phone after. Pywell stated that she was operating her Toyota Prius bearing MA reg. # 2197RW on Webster Street facing westbound at Cherry Street waiting for the green signal to turn left on to Cherry Street. Once the light changed, she attempted to turn and never saw a pedestrian trying to cross Cherry Street. I asked her if she ever felt contact to which she replied that she was not aware that she ever struck someone. She further says that once she did see him, she believed that he never lost his balance or ever fell down. She says that he retrieved his backpack from the ground and approached her driver's side opening her door stating that he was struck by her.

On 06/10/2020 at 1249hrs I responded to the intersection of Fairway Drive and Watertown Street for a two car motor vehicle crash involving a city of Newton Fire Department vehicle. Vehicle 1 described as a 2009 Ford Taurus Sedan (MA Municipal Fire Reg 64260) was backing southbound on Fairway Drive when it struck Vehicle 2 which was unoccupied and parked in the southbound lane of Fairway Drive. On Wednesday 6/17/2020 at approx 2116hrs, while assigned to N-494, I responded to the intersection of Walnut St and Comm Ave for MVA involving a bicyclist. The bicyclist, Aziz Boltayev, said he was traveling NB on Walnut St and attempted to cross Comm Ave. He stated that he was in the middle of the intersection when Comm Ave' light signal turned green. Contact was made to MV1s driver side front panel. Boltayev was transported to Beth Israel with serious injuries to his legs. Boltayev was not wearing a helmet nor wearing reflective clothing. His bicycle, a blue Schwinn, had no front head lamp attached in violation of MGL c85 s11B. A citation was issued and mailed to Boltayev. He was wearing a light colored shirt and shorts. Operator of MV1 states that he was on Comm Ave heading WB and stopped at a red light at the intersection of Walnut St. He states that he had a green signal when attempting to cross the intersection and did not see the bicyclist prior to the accident. Pictures were taken and submitted to IT. The Schwinn bike was placed in property and evidence for safekeeping.

On Thurs 7/9/20 at approximately 1911 hrs I responded to 111 Dedham St for a report of an accident involving a bicyclist. Upon arrival I observed the MV and the bicycle on the EB travel lane side of Dedham St with the bicycle on the sidewalk/driveway and the MV facing the driveway at 111 Dedham St. I spoke with the operator of MV#1 who stated that he was traveling EB on Dedham St and then turning right into his driveway when he collided with the bicyclist. He stated that he never saw her until the last second as the bike was in his blind spot. When contact was made he immediately got out of his car to see if the bicyclist was okay. The passenger in the vehicle stated that the bicyclist was traveling Eastbound on Dedham St on the sidewalk when MV#1 struck her. The bicyclist was riding a turquoise and black Bianchi Aria racing bike with black leggings and a white and blue tank top. The bicyclist was wearing a helmet which had little to no damage on it. Before being transported to BI by the medics, Ms. Loomis stated she was traveling EB on Dedham St in the shoulder/bike lane when she was struck by MV#1 who was attempting to turn into the driveway. Pictures were taken of the scene and the SIM card was turned over to the IT bureau to be uploaded into this report. The bike was turned over to the bicyclist's friend, Christine Long.

On 7/21/20 at approx 1615 hrs I responded to the area of Elliot St for a report of a MVA where a vehicle had struck a pole and took off. Upon arrival I encountered a 10 wheeler truck that had struck a city tree. The truck was traveling Westbound on Elliot St when it clipped a city tree located on the sidewalk across from Wetherell St. There were no injuries on scene and the truck had roof damage to the passenger side of the vehicle. A private tow came to tow the truck. Pictures were taken of the accident and turned over to IT to be uploaded.

On 08/09/2020 at 1500 hrs, I was dispatched to the area of Chestnut St. and Gordon Rd. for a Motor Vehicle Accident. Dispatch stated a UHaul truck had collided into a city tree. Upon arrival, I observed a Uhaul truck, on the sidewalk in front of 458 Chestnut St. The truck was facing northbound on Chestnut St. and had heavy front end damage as it was driven into tree #2.The operator stated that tree #1 had fallen onto the road, causing her to swerve, hit the curb and then collide into tree #2.. There were tire marks leading from the curb tree #2. The Newton Forestry Department was contacted to remove the tree #2 from the roadway, and to evaluate tree #1. (Both city tree's). Tody's Tow service arrived on scene and removed the Uhaul truck from the sidewalk. The operator stated she was not injured and declined medical attention. The homeowner at 458 Chestnut St. was give instructions on how to obtain a copy of this report, as potential damage was done to her property. Pictures of the damaged Uhaul truck, and both tree's were taken on the (N493) camera and submitted to the Newton Police It Bureau.

On 08/12/20 at approximately 13:53 I responded to the intersection of Charlemont St and Roland St for a truck that had taken down some wires. Upon arrival I saw a Harvey Building truck in the middle of the intersection with power and cable lines resting on its cab. I spoke with the operator/employee Bernard McLain who stated he was traveling on Charlemont St and attempted to take a right in the middle of the road as to avoid the low hanging wires. As he took the right, McLain realized he had started to pull on the wires and immediately stopped. Verizon pole #602/10 along with its wires were pulled down and the pole is presently leaning. Verizon and Eversource were notified to respond. At approximately 15:30 a Newton Police Detail relieved me to take over the scene. Photos of the scene were taken and emailed to our I.T. Bureau for attachment to this report. There was no apparent damage to the truck.

On 8/18/20 at 1548 hrs I responded to Sutcliffe Pk for a Waste Management truck that had taken down a telephone pole. The truck was turning around at the top of Sutcliffe Pk and got caught in the wires and an overhanging tree. As the truck continued to progress it dragged more of the wires and snapped a telephone pole causing it to fall on the street. The operator states that the truck is 13' 6" high. 5 pictures were taken of the scene and were turned over to IT. No injuries reported on scene. Eversource responded and restored power. Eversource, Verizon, and Comcast all had wires damaged due to the accident.

On August 19th, 2020, while working in marked unit N526, I responded to a MVA vs. bicyclist in the area of the intersection of Crafts Street and Watertown Street, Newton. The weather at the time of the crash was clear and cloudy. The road surface was dry. Crafts Street and Watertown Street are both public ways maintained by the City of Newton. The operator of MV1, Cody Hjelm (NHL19517660), stated he was traveling westbound on Watertown Street and had just crossed over Crafts Street when he saw a bicyclist traveling in the same direction on Watertown Street. As MV1 approached the bicyclist he moved over to the left towards the double yellow lines to give the bicyclist room. MV1 states the bicyclist was no longer in front of his vehicle when he heard a thud type noise come from the side of his van. MV1 stated he pulled over and saw the bicyclist on the ground with a head injury. MV1 sustained moderate damage to the right passenger side of the vehicle. The operator of MV1 reported no injuries and signed a patient refusal. MV1 was able to leave the scene of the accident. The bicyclist identified as Harriet Fell (S04980771) was being treated by medics on scene at the time of my arrival. I spoke with her briefly inside the ambulance as she was being treated for a head injury. Harriet stated she has no recollection of the accident and does not remember what happened before or after the crash. Medics advised she was being taken to NWH for treatment. I spoke with Harriet on the telephone around 1630 hours. She stated she still does not remember what happened but stated she was just finishing her forty mile bike ride and was heading home. Harriet stated she has a hematoma on her head and recieved three stiches. Harriet also stated she has small abrasions on both hands. Harriet stated she was being released from the hospital shortly and will pick up her bicycle tomorrow. The bicycle was transported to the police station by Newton DPW and was secured for safekeeping inside Support Services by Officer Manouk. A property form was completed and submitted along with an Incident report # 20039014. I took photographs of the accident scene and submitted them to the IT Bureau. Update by Ofc. Geagan: On 08/20/2020 Ms. Fell came to the Support Serices Bureau to retrive her bicycle. See attached Evidence/ Property Form and Return of Property Sheet

On Tuesday August 25th, 2020 at approximately 0938 while in N526 I responded to Gordon Rd and Paulson Rd for a City of Newton vehicle that took down Verizon wires. Gordon Rd and Paulson Rd are both owned and maintained by the City of Newton. At the time of the accident the weather was clear and.. sunny and the road surface was dry. On scene was Officer Gabriel who reported that the operator of the 2018 Case Backhoe Tractor identified as Greg Wansiewicz (S81742023) stated that he was traveling on Gordon Rd and turned left onto Paulson Rd when the top of the Backhoe took down low hanging wires. The operator stated the wires were too low and the bucket on the machine was down at the time.The operator reported no injuries and there was no damage to the city vehicle. Pictures of the scene were taken and submitted to the IT Bureau. Verizon was notified and were on scene as well as several DPW vehicles. DPW set up barricades to block off the area until Verizon arrived. The wires that were damaged were from Verizon pole 910/8 to Verizon pole 1002/2.

Operator of Motor Vehicle #1 stated he was in his truck traveling Westbound on Washington Street and struck a large City of Newton tree branch that was hanging over the road. The city tree which was directly across the street from 119 Washington Street was ripped out of the ground next to the sidewalk and split into two large pieces causing Waverly Avenue to be blocked. City of Newton forestry came out and assessed the scene and Northern Tree arrived to clear out the branches in the middle of the road. Motor Vehicle #1 sustained minor front upper damage. Several photographs were taken and the SD Card was placed in the I.T. Bureau to attach photos to this report.

On Tuesday, September 1, 2020, while assigned to marked unit N496, I responded to the area of 780 Beacon Street, Newton for a report of a motor vehicle crash involving a bicyclist. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of the vehicle involved in the crash, Dina Hudson (S33833807). Ms. Hudson stated she was sitting in her 2015 white Jeep Grand Cherokee (MA: 727SZ7) that was parked (E) in a metered parking spot in front of 780 Beacon Street. Ms. Hudson stated traffic in the Eastbound lane of Beacon Street to her left was stopped. There was a red light ahead at the intersection and traffic had built up past where her vehicle was parked. Ms. Hudson stated she opened her driver side door while she was looking for her debit card to go to the bank. Ms. Hudson stated at this time a bicyclist was traveling at a high rate of speed around the stopped traffic towards the intersection and crashed into her driver side door. Ms. Hudson stated the operator of the bicycle then traveled further up the roadway and crashed behind a vehicle stopped in traffic. Ms. Hudson stated she was unsure if the bicyclist crashed into the stopped vehicle or if it had landed on the roadway just before it. Ms. Hudson stated the bicyclist then got up from the roadway and began yelling at her that the crash was her fault. I observed a few small fresh scrapes on the outside of the driver side door area. When I initially arrived at the crash location, I observed Officer Lazarakis checking on his well-being of the injured bicyclist, Marc Lind. Lind was observed laying on the sidewalk in front of 780 Beacon Street. After being evaluated by Newton Medics, I spoke with Mr. Lind. Mr. Lind stated he was traveling on his Trek bicycle Eastbound on Beacon Street towards Langley Road. Mr. Lind stated while traveling through the area at this time, traffic in the Eastbound Lane was stopped and there was a red light ahead of him at the intersection. Mr. Lind estimated his speed at this time to be 10-12 miles per hour. Mr. Lind stated as he travelled past the Santander Bank at 780 Beacon Street, the operator of a parked white Jeep to his right abruptly opened their door into traffic. Mr. Lind stated he attempted to avoid the vehicles door and veered to the left. Mr. Lind stated the right end of his handlebar made contact with the door and as he traveled forward and applied his brakes his right side crashed into the rear of a vehicle stopped in traffic ahead of him. Mr. Lind stated he crashed to the roadway on his right side. Mr. Lind was able to move to the side of the roadway and wait for Newton PD, Fire, and Medics to respond. Mr. Lind reported injuries to his right arm, side, and leg area. Mr. Lind signed a patient refusal with Newton Medics and was transported away from the area by his wife with his bicycle. I observed no substantial damage to his Trek bicycle. At the time of the crash, Mr. Lind was wearing a white t-shirt, black shorts, black, shoes, and a gray helmet with sunglasses. I spoke with a witness to the crash, Steve Worthington. Mr. Worthington stated he was stopped in traffic on Beacon Street about 4 vehicles back from the red light. Mr. Worthington stated he looked out of his passenger door mirror and observed a bicyclist "whipping" down Beacon Street in between vehicles parked on the side of the road and vehicles stopped in traffic at the red light. Mr. Worthington estimated the speed of the bicycle to be approximately 20 miles per hour. Mr. Worthington stated the bicyclist had to "lean" by his vehicle to get by. Mr. Worthington stated a vehicle just ahead of him that was parked opened their door and the bicyclist made contact with the door. Mr. Worthington stated the bicyclist then went down in front of the car that was in front of him. Mr. Worthington stated the bicyclist was rolling around on the ground and screaming "It's your fault" at the woman that opened her door Photos were taken of the bicycle and the vehicle involved and submitted to the IT Bureau.
