Narrative:
Operator of MV1 was driving westbound on Beacon St in the area of 741 Beacon. Operator of MV1 stated his tire blew out and the car swerved into the side of MV2 and then the rear of MV3 which were both parked legally. Witness 1 stated she heard the crash but did not see the impact. She stated she heard the crashing noise first, and then heard a loud popping noise of the tire blowing out shortly after. Witness 1 identified Cesar Ramirez Rodriguez as the Operator of MV1. Operator of MV1 was uninjured, and the other two vehicles were unoccupied. Todys responded and towed MV1 and MV2. Operator of MV1 was issued, in hand, MA citation T1440439 for 90/23 Operating after suspension and 89/4A marked lanes violation. I was eventually able to locate the owners/drivers of the parked vehicles and they were given the report number. Beacon St is a public way in the City of Newton.

Vehicle 1 was travelling straight ahead, westbound on Watertown St. Vehicle 2 was in the left turn only lane, and taking a turn onto Crafts St. The operator of vehicle 1 stated they needed to swerve off of the road to avoid getting struck by the rear of vehicle 2. Vehicle 1 struck the curb with their right front tire. The tire popped and the rim was damaged. No one was injured due to the accident. Vehicle 1 was towed by Tody's. A level 3 inspection was done to vehicle 2 by Officer Marini, and a citation was issued.

On Wednesday, February 13, 2019 while assigned to Traffic unit N525, I responded to the 280 Newtonville Avenue, Newton for a report of a past hit and run accident. I met with Madeleine Aubourg (S67566148) who stated she is a Nursing Assistant at Cabot Park Village. Aubourg stated due to the snowfall the area had the previous day, Aubourg left her vehicle parked (S) in the rear parking lot overnight. Aubourg stated this morning when she returned to work she observed damage to the front bumper area of her 2018 Toyota Camry (MA: 6AN179). Aubourg stated she believes sometime overnight when the parking lot was being plowed a plow truck hit the front of her vehicle. I observed moderate damage to the front bumper area. I spoke with Adolfo Merida-Diaz (S44133458) on scene who stated he is employed by Marquense Landscaping and is responsible for the snow removal at 280 Newtonville Avenue. Merida-Diaz stated last evening and this morning he removed snow from the parking lot and did not make contact with any vehicles in the parking lot during his duties. I showed Merida-Diaz where the alleged accident occurred. Merida-Diaz stated he does not remember a car being parked in that particular spot, but if there was a car parked there he leaves enough room so he is not near any vehicles. I did not observe any surveillance cameras in this portion of the parking lot. There are no witnesses to the alleged accident. Aubourg was advised to speak with her insurance company.

Narrative:
On 3/1/19 while working N493, myself and N492 Officer Selig were sent to the area of 1354 Washington St for a motor vehicle accident with reported injuries. On arrival, I spoke to Matthew Digregorio DOB7/2/74 ( hereby referred to as operator #1) who was standing on the sidewalk next to MA reg 134TR9 (vehicle #1 ). Vehicle #1 had significant damage to the front and right quarter panels. It was straddled between the travel lanes and did not have the keys in the ignition. It was immediately apparent that operator #1 suffered an injury to his head, as there was a moderate amount of blood and small pieces of glass stuck in his forehead area. I observed operator #1 to be unsteady on his feet, have bloodshot eyes and slurred speech. I could detect a moderate odor of alchoholic beverage coming from operator #1 as I was approaching him, which was apparent from about an arms length away (it should be noted that medics were already en route to the scene at this time). Prior to the medics arrival, I asked operator #1 for a brief synopsis of what transpired. Operator #1 stated that he had just left Paddy's Pub and was driving westbound on Hunter St (which is approximately 1/8 mile from our location) when he collided head on with vehicle #2. Vehicle #2 was not immediately visible upon police arrival, however was pointed out as being located at the intersection of Hunter and Putnam St. I asked operator #1 where he was heading, as Hunter St is a back road not directly in the vicinity of Paddy's. Operator #1 could not answer the question and also remained silent when I asked him how much alcohol he had consumed this evening. At this time the medics arrived on scene. I asked operator #1 for the car keys to reliquish to the tow truck operator, which is standard operating practice. Operator #1 reached into his pocket and then provided me with the keys. Due to concern over operator #1's obvious head trauma, he was then transported to NWH to be evaluated while the investigation continued on scene. Two other parties approached myself and Officer Selig. One was the operator of vehicle #2, MA reg 1RD999. Operator #2 is identified as Luke Ferdinands DOB 5/11/80. The other party who approached is identified as Douglas Dasilva-Pimenta DOB 6/2/86 (Dasilva-Pimenta will now be referred to as operator #3). Operator #2 states that he was taking a right onto Hunter St from Putnam St, when he observed vehicle #1 on the wrong side of the road approaching him head on. Operator #2 states vehicle #1 then collided head on with him, took a right onto Putnam St, and continued onto the Washington St overpass near 1354 Washington St. Operator #3 states he was inside of #5 Hunter St when he heard a loud crash coming from outside. He looked out the window and saw vehicle #1 traveling at a high rate of speed on Hunter St towards Putnam St. Operator #3 observed damage to the right side of his parked vehicle (now referred to as vehicle #3) MA reg 7CAK80. He immediately got in his vehicle and proceeded to follow vehicle #1, which then crashed into vehicle #2 head on. Operator #1 did not stop until it was obvious that operator #3 was following close behind. This ultimately landed vehicle #1 at it's final location near 1354 Washington St. Vehicle #1 and vehicle #2 were not driveable and were towed to Tody's. It should be noted that there was a passenger in vehicle #2 at the time of the collision. Airbags were deployed and medics checked out both operator and passenger. Both declined transport. The passenger is identified as Elizabeth Ferdinands DOB 5/22/53.Ferdinands' version of events coincide with that of operator #2. Upon speaking with Officer Selig it was learned that operator #1 dropped his cell phone several times as he was holding it on scene. Both Officer Selig and operators #2 and #3 observed this. Operators #2 and #3 both expressed concern over the obvious odor of alcohol eminating from operator #1, and the fact that he was unsteady on his feet. I responded to NWH to follow up with operator #1 however he did not wish to speak further. He did express concern over the well being of all parties involved. Operator #1 was issued in hand the following citations: T1268426 - 90/24 OUI Alcohol, 90/24 Leaving the Scene (property damage, vehicle #2), and 90/24 Leaving the Scene (property damage, vehicle #3). T1268425 - 89/4a Marked Lanes violation. T1269216 - 90/24 Negligent Operation of a M/V (mailed after consultation with superiors) All streets mentioned are public ways in the City of Newton. 1354 Washington St will be the address used on this report and subsequent citations due to multiple locations being involved. Citations and paperwork have been placed in the prosecutor's mailbox. An immediate threat form has been filled out and faxed to Mass DOT. *Attention Police Prosecutor for blood work subpoena, if applicable*

On 3/4/19 I responded to the area of Hammond St and Chestnut Hill Rd for a report of a two car MVA. Upon arrival it was determined there were no injuries and the two drivers signed patient refusals with the ambulance. I first spoke with the operator of MV#1 who stated he was driving northbound on Hammond St near the Chestnut Hill T station where it bends and MV#2 was coming southbound and skidded on the snow and came into his lane. I then spoke with the operator of MV#2 who stated he was driving southbound on Hammond St when his vehicle lost control around the turn and drifted into the opposite lane and struck MV#1. I issued the operator of MV#2 MA Citation T1268628 for Chapter 89/4A; Marked Lanes Violation. At the time of this accident there were heavy snow conditions. Tody's Towing responded and towed both vehicles from the scene.

On 3/4/19 at approximately 1630hrs, I responded to the front desk for a report of a past hit and run. Operator#1 stated she was involved in an an accident on 2/27/19 at 1436hrs in front of Mango Thai, 115 Elm St. Operator #1 stated she was stopped waiting for the red light at Elm St and Washington when a "new black or gray SUV" pulled out of a parking spot the right of her. The other vehicle sideswiped her passenger side causing a large scratch. Operator #1i stated she honked and attempted to stop the vehicle but the driver pulled away. Operator #1 was only able to identify the driver as a male but did provide me with a plate of MA Reg 9RM445. The plate comes back leased to a 2018 Gray Jeep Compass registered to a Matthew Veteto. I was unable to find any phone number for Veteto. Operator#1 has already filed a claim with her insurance company who advised her she would need a police report. Due to being unable to verify who was driving, a citation was not issued.

On Monday, March 11, 2019 while assigned to Traffic unit N525, I responded to the area of Watertown Street and Adams Street, Newton for a report of a motor vehicle/pedestrian accident. The accident was reported to Newton Police Dispatch by Newton Police Officer Charles Guarino. Officer Guarino identified the pedestrian involved in the accident as Newton Police Officer James Selig. Watertown Street and Adams Street are both public ways maintained by the City of Newton. After completing my investigation, the operator of MV1, Colleen Butaney (S20012566) was issued Massachusetts Uniform Citation T1272995 for Chapter 90, Section 25 (Fail to Stop for a Police Officer) and Chapter 89, Section 4A (Marked Lanes Violation). Butaney was advised and the citation was placed in her property in Locker #4 in the NPD holding area prior to her release. Officer T. McCarthy completed and submitted a criminal application for Butaney for Chapter 265, Section 15b (Assault with a Dangerous Weapon, MV). Please refer to incident report #19009204 for statements from all parties involved. Photos were taken of the area/vehicle and submitted to the IT Bureau. The vehicle involved, (MA: 531PX6) was removed from the roadway by Tody's towing. Officer Marini completed a towed motor vehicle report form. Surveillance footage of the incident was recovered from a local business and was submitted to the IT Bureau/Support Services. A property and evidence form was completed and submitted to Officer Manouk.

OPERATOR #2 REPORTS THAT SHE WAS IN THE FOURTH LANE ON THE CENTRE ST BRIDGE WHICH IS FOR TRAFFIC TO EITHER TURN LEFT TO WASHINGTON ST OR FOR STRAIGHT AHEAD FOR CENTRE ST. SHE WAS TURNING LEFT TOWARDS WASHINGTON ST W/B WHEN VEHICLE #1 WENT STRAIGHT AHEAD CAUSING THE CRASH. VEHICLE #1 WAS IN THE WRONG LANE FOR THE DIRECTION SHE WAS HEADING. VEHICLE #2 WAS IN THE PROPER LANE FOR THE DIRECTION SHE WAS GOING.

On Thursday, March 28, 2019 while assigned to Traffic unit N525, I responded to the area of Dedham Street and Bound Brook Road, Newton for a report of a motor vehicle accident. Dedham Street and Bound Brook Road are both public ways maintained by the City of Newton. Anna Johnson (S14645384) stated her vehicle, a 2012 Honda CRV (MA: 4AG988), was parked unoccupied on Dedham Street (S) right before Bound Brook Road. Johnson stated she was made aware that her vehicle was hit by a passing vehicle. When she arrived at her vehicle, she observed damage to the driver side door mirror and the rear driver side door area. A note with a name and phone number was left on her vehicle by the operator of MV2. I spoke with Maria Diaz-Santos (S91121221) stated she was operating her 2018 Chrysler Pacifica (MA: 1MMB61) south on Dedham Street towards Bound Brook Road. Diaz-Santos stated as she was traveling past MV1, her rear passenger side door mirror area made contact with the driver side of the parked vehicle. Diaz-Santos stated she contacted Newton PD to make them aware of the accident and left her name and phone number on MV1 for it's owner to exchange information. Diaz-Santos stated there is very minor damage to her passenger side door mirror.

On Wednesday April 10th, 2019 at approximately 0755 hours I responded to the area of Centre Street and Washington Street for two car motor vehicle accident involving a Newton school bus with children on board. Centre Street and Washington Street are both public ways in the City of Newton. At the time of the accident the weather was cloudy with light rain. The road conditions were fairly dry. I spoke to the operator of MV1 identified as Julie Lym (S16060051) who stated she was traveling northbound on Washington Street onto the Centre Street bridge. MV1 stated she was attempting to cut across lanes to merge on to the Mass Pike Westbound. MV1 stated traffic was at a standstill and she saw an opening to change lanes. MV1 stated MV2 was stopped in front of her and as she attempted to change lanes her vehicle side swiped the rear drivers side corner of the school bus. The operator of MV1 reported no injuries. MV1 sustained minor damage and was able to leave the scene. I spoke with the operator of MV2 identified as Antonio Mendes Goncalves (S54275868) who stated he was stopped at the red light in the right middle lane on the Centre Street bridge. MV2 stated MV1 attempted to change lanes and struck the rear drivers side corner of the school bus. MV2 sustained minor damage and the operator reported no injuries. MV2 is a City of Newton contracted yellow school bus that had six children present on the bus at the time of the accident. The school bus was transporting th e children to Bigelow Middle School. All children present reported no injuries and their parents were notified by the school administration. I escorted the school bus back to the school where the children were all evaluated by the school nurse. 14 photographs were taken and submitted to the IT Bureau.

MV#1 stated while traveling Southbound on Centre St in the left turn lane at the intersection of Centre and Commonwealth ave. He stated he was at a complete stop waiting to turn left on to Commonwealth ave. He stated MV#2 was in the right lane adjasent to his vehicle when it took a sudden left turn on to Commonwealth Ave striking MV#1 in the right rear side oh his vehicle with the front left end of MV#2. He stated MV#2 then continued to drive behind him Eastbound on Commonwealth ave. MV#1 stopped on Read CT to evaluate the damage at which point MV#2 kept driving. MV#1 stated the car traveling behind MV#2 witnessed the incident and gatherd the plate information. The witness was not on scene upon my arrival nor did MV#1 gather their information. Newton Dispatch ran MA Reg 7EW346. The plate came back to a Black Honda Civic 2005 matching MV#1's description of the vehicle. The owner of the Civic was listed out of Brighton. I followed up at 78R Foster St Brighton, Upon arrival I noticed a black Civic matching the plate and description. The Civic had damage to the front end and damage to the back right side. I spoke with owner of the vehicle that stated he had not been driving today and had been at home. He did not know who was operating the vehicle at the time. Speaking with Medina the owner of MV#2 there was a significant language barrier issue. I will not be charging owner of MV#2 due to the vehicle damage not being consistent with MV#1 statements, as well as proof of operation. MV#1 was not towed. Operator of MV#1 stated he did not sustain any injuries at this time. Owner of MV#2 did not have any injuries to him consistent with being in a MV accident.

ON THIS DATE I RESPONDED TO 5 MEADOWBROOK RD, FOR A REPORT OF AN AUTO ACCIDENT (SEE INCIDENT REPORT #19018289). THE CALL WAS RECEIVED FROM THE ENVIRONMENTAL POLICE WHO WERE ON-SCENE. ON-SCENE PRIOR TO MY ARRIVAL WERE CAPTAIN DOWLING AND TRAFFIC SERGEANT LEE. CATALDO MEDIC 2 AND AMBULANCE 1 ARRIVED ON-SCENE, SHORTLY AFTER I DID. UPON MY ARRIVAL, I OBSERVED A SILVER HONDA CIVIC WITH HEAVY FRONT END DAMAGE. THE VEHICLE DISPLAYED MASSACHUSETTS REGISTRATION 7ZF784. TWO PEOPLE WERE STANDING BESIDE THE VEHICLE. A MAN AND A WOMEN SAID THEY HAD BEEN IN THE CAR. THE MAN, LATER IDENTIFIED AS BENJAMIN SMALL STATED HE HAD BEEN OPERATING THE VEHICLE. THE WOMAN, LATER IDENTIFIED AS KIMBERLY OAKES, STATED SHE WAS A PASSENGER IN THE VEHICLE. I ASKED THEM IF THEY WERE OK OR NEEDED MEDICAL HELP AND THEY SAID THEY WERE NOT INJURED. THE MEDICS CHECKED OUT MR SMALL ( MEDICS ELISE GADANSKY AND ROBERT ANDERSON) WHICH INCLUDED CHECKING FOR LOW BLOOD SUGAR, DUE TO HIS INABILITY TO SPEAK CLEARLY AND LEGIBLY. HIS BLOOD SUGAR CHECKED OUT FINE. I THEN ASKED MR SMALL WHAT HAD HAPPENED. HE STATED THAT HE WAS TRAVELING WESTBOUND ON DEDHAM ST WHEN ALL OF A SUDDEN A WHITE MINI VAN CROSSED OVER THE DOUBLE YELLOW LINE AND WAS HEADING TOWARDS HIM. HE STATED THAT IN ORDER TO AVOID A HEAD ON COLLISION, HE SWERVED RIGHT, OFF THE ROAD AND STRUCK A TELEPHONE POLE (LATER DETERMINED TO BE UTILITY POLE #60 LOCATED IN FRONT OF 556 DEDHAM STREET). HE STATED THAT HE PULLED OVER ONTO MEADOWBROOK RD. BOTH DEDHAM STREET AND MEADOWBROOK RD ARE PUBLIC WAYS IN THE CITY OF NEWTON. AS I WAS TALKING TO MR SMALL HE APPEARED TO SPEAK VERY SLOWLY, HE APPEARED TO BE LETHARGIC, BUT I COULD NOT DETECT ANY ODOR OF AN ALCOHOLIC BEVERAGE ON HIS BREATH. NEXT I SPOKE TO MS OAKES WHO STATED SHE IS MR SMALL'S GIRLFRIEND. SHE STATED SHE WAS A PASSENGER IN THE FRONT SEAT OF THE CAR. SHE ALSO STATED THAT MR SMALL WAS OPERATING THE VEHICLE WHEN A VEHICLE HAD CUT THEM OFF FORCING THEM OFF THE ROAD INTO THE TELEPHONE POLE. DUE TO THE FACT THAT WE COULD NOT SMELL ANY ALCOHOLIC BEVERAGE ON HIS BREATH, CAPTAIN DOWLING REQUESTED THAT A DRUG RECOGNITION EXPERT (DRE) BE CALLED TO THE SCENE. DISPATCH NOTIFIED SURROUNDING CITIES AND TOWNS OF OUR NEED FOR A DRE. NOBODY WAS ABLE TO RESPOND. TRAFFIC OFFICER WADE RESPONDED TO THE SCENE AND PERFORMED A ROADSIDE ASSESSMENT ON MR SMALL. SEE HIS SUPPLEMENTAL REPORT. BASED ON OFFICER WADES ROADSIDE ASSESSMENT AND MY ON-SCENE OBSERVATIONS, I PLACED MR SMALL UNDER ARREST FOR OUI DRUGS, MARKED LANES, AND NEGLIGENT OPERATION OF A MV. MR SMALL WAS HANDCUFFED AND SEARCHED BY OFFICER WADE. PRIOR TO TRANSPORT, CRUISER 499 WAS SEARCHED BY OFFICER WADE FOR CONTRABAND WITH NEGATIVE RESULTS. OFFICER WADE THEN PLACED MR SMALL INTO CRUISER 499 AND SEAT BELTED HIM. WHILE OFFICER WADE DID THIS, OFFICER WILSON AND I DID AN INVENTORY OF THE VEHICLE. DURING THE INVENTORY, 9 CVS PAPER PRESCRIPTION PRINTOUTS WERE FOUND. THEY WERE ALL IN MR SMALL'S NAME FOR THE DRUG "ADDERALL" (8) OR "KLONOPIN" (1). ALSO FOUND WAS A SMALL YELLOW PLASTIC PRESCRIPTION TYPE BOTTLE CONTAINING AN UNKNOWN LIQUID. OFFICER WILSON PLACED THE BOTTLE OF UNKNOWN LIQUID INTO A BAG AND HAD IT LATER EXAMINED AT THE NPD STATION BY DET. PICTURES OF THE VEHICLE AND DAMAGED TELEPHONE POLE WERE TAKEN BY OFFICER WILSON. OFFICER WILSON TURNED THE DISC OVER TO DIRECTOR SMITH, WHO DOWNLOADED THEM. AFTER THE MV INVENTORY, I TRANSPORTED MR SMALL TO NEWTON POLICE FOR BOOKING. AT THE STATION MR SMALL WAS REMOVED FROM MY CRUISER AND THE CRUISER WAS AGAIN SEARCHED AND I FOUND NO CONTRABAND OR WEAPONS. HE WAS THEN PROCESSED BY OFFICER JASON SCHLEGEL, BOOKED BY SGT O'CONNELL, THEN PLACED INTO CELL #3. MR SMALL WAS ISSUED CITATION NUMBER T0645459, IN HAND FOR 1.) OUI DRUGS CH90 SEC24, 2.) MARKED LANES VIOLATION CH90 SEC4A, 3.) NEGLIGENT OPERATION CH90 SEC24E. AN IMMEDIATE THREAT FORM WAS FILLED OUT BY MYSELF AND A COPY OF THE REPORT ALONG WITH THE MOTOR VEHICLE CITATION WERE FAXED OVER TO THE MASSACHUSETTS REGISTRY OF MOTOR VEHICLES.

On Thursday, May 16th 2019, at approximately 6:40am, I, Officer Brooks, responded to 930 Watertown street for a report of a head on collision. Upon my arrival I spoke with the operator of MV1(MA REG RT91EL) who stated she was traveling westbound on Watertown street and "blacked out" and could not recall how the accident happened. I asked what she meant about blacking out, and she stated she thought she had a panic attack and may have passed out. The operator of MV1 stated she takes Lorazepam for panic attacks, but has never had one like this. MV1 had heavy front end damage and airbag deployment, specifically to the driver's side. The operator was uninjured. I then spoke with the operator of MV2(RI COM 81082) who stated he was traveling eastbound on Watertown street, and observed MV1 crossing over the center line. The operator of MV2 stated he tried to get out of the way but MV1 continued into his lane and struck him head on. MV2 had damage to the drivers side front end. The operator stated that he was suffering from shoulder pain to his right shoulder. The operator of MV2 was transported to Newton Welleseley by Cataldo. Both MV1 and MV2 were towed by Tody's due to the damage. As a result of the collision, MV2 rolled into parked a vehicle MV3(MA REG 315PW6). There was minor damage to the rear end of MV3. The owner was notified and provided the crash information. The operator of MV1 was mailed MA Uniform citation T1442362 for 89/4a Marked Lanes violation, and additionally based on her statement of passing out at the wheel I filed an immediate threat with the registry. The operator of MV2 was mailed MA Uniform Citation T1442363 for 90/9 Expired registration.

On 5/20/19 at about 1510hrs I was dispatched to 136 Evelyn Rd for an MVA. En route to the location with Ofc Gourdeau, it was determined by first arriving Ofc Gourdeau that a vehicle had crashed through a fence at the end of Troy Lane and came to rest in the backyard of 126 Evelyn Rd. On arrival, I located the vehicle operator, identified as Mr Stuart Phillips who was operating MA Reg 8VVP40, an '03 Jaguar S Type model. Mr Phillips was walking around the backyard after the collision and appeared coherent as to what had transpired. He sustained a laceration to the top of his head and was bleeding heavily. We sat him down in a lawn chair prior to medics arrival to evaluate. When asked what happened, he stated he "hit the gas instead of the brakes". I asked him where did the accident begin? He stated he was turning around at the end of Troy Lane (a dead end cul de sac) and crashed through the fence there. Mr Phillips was alert and speaking clearly. I asked him if he was on any medication? He stated he was taking Plavix only, a blood thinner. I asked about his medical history and he stated he was released from Spalding Rehab today for treatment from a stroke and was receiving occupational therapy. Medics arrived to evaluate and he was transported to NWH for treatment. An investigation of the crash path of the vehicle revealed the following based on tire tracks and damage left behind: The vehicle crashed through the fence at the end of the cul de sac belonging to 41A Helene Rd; Vehicle drove over that back lawn, clipped the back deck attached to the rear of the house; damaging the base of the deck and lattice below; Vehicle travelled over the patio, then crashed through shrubbery down an embankment leading to the backyard of 132 Evelyn Rd; Vehicle drove through backyard of 132 Evelyn, then continued along the side of the house where it drove over damaging three retaining walls and clipped a large tree; After driving over the third retaining wall, vehicle continued through the backyard of 126 Evelyn, causing significant lawn damage; Vehicle continued until it hit the back of the house at 126 Evelyn; there, it drove over the rear patio before it struck a ground level basement window and window well; It came to a rest there where the vehicle caught on fire under the hood. Witnesses helped extinguish the fire with a garden house (see incident report #19020129) until NFD arrived to knock it down. Significant front lawn damage to 126 Evelyn occurred as well as NFD was arriving on scene to what was first reported as a structure fire. Several photos of the area were taken by myself. The memory chip was TOT the I.T. bureau to be attached to this report. Tody's towing towed the vehicle out of the backyard by accessing the rear driveway of 132 Evelyn Rd. Mr Phillips cited for Negligent Operation and an Immediate Threat for License Suspension/Revocation faxed to the RMV. Troy Lane is a public way in the City of Newton.

On Wednesday, May 22nd 2019, at approximately 6:53am, I, Officer Brooks, responded to Crafts street at Albemarle road for a report of a 2 car MVA. Upon my arrival I spoke with the operator of MV1(MA REG 9MJ713 ), who stated he was traveling northbound on Albemarle road. He stated he stopped at the stop sign at the intersection with Crafts street, and traffic traveling Eastbound on Crafts street stopped to let him cross over Crafts street and continue on Albemarle road. As he proceeded through the stop sign, he was struck on the drivers side front end by MV2(MA REG 7HJ262), which had crossed over the center line of Crafts street traveling Eastbound, passing the cars which had stopped to let MV1 out. MV1 had damage to the drivers side front quarter panel and front bumper. The operator was not injured. I then spoke with the operator of MV2 who stated she was traveling Eastbound on Crafts street, and was attempting to take a left hand turn onto North street, so she was driving in the center lane. The center lane the operator is referring to is actually a left hand turn lane for vehicles traveling Westbound on Crafts street for vehicles who wish to turn onto Albemarle road. There is no right of way in that center lane for vehicles traveling Eastbound. MV2 had damage to the passenger side front quarter panel. The operator was uninjured. The operator of MV2 was issued a citation in hand for 89/4A Marked lanes violation. She notified AAA for a tow. MV1 was towed by Tody's. Both operators were provided the crash report number.

Upon my arrival I observed that MV1(MA REG 574PF5) had struck MV2(FL REG CWWE22) which was parked legally in front of 163 Lexington street. I spoke with the operator of MV1 who stated he was traveling southbound on Lexington street, and had reached over to his passenger side to grab a Burger King bag. When he reached for the bag he drifted to the right and struck MV2. MV1 had heavy damage to the passenger side and airbag deployment. The operator was checked out by Cataldo and signed a patient refusal. MV1 was towed by Tody's. The operator was issued a citation for an expired registration. I spoke with the owner of MV2 who was inside of 163 Lexington street at the time of the accident. His vehicle had been legally parked when struck. MV2 sustained major damage to the driver's side. MV2 was towed by Tody's. Both operators were provided the report number.

On Monday, July 1, 2019 while assigned to Traffic unit N525, I responded to the area of 802 Walnut Street, Newton for a report of a motor vehicle accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Walnut Street is a public way maintained by the City of Newton. The operator of MV1, Dayana Martinez (S96710554), stated she was operating her 2019 Toyota Rav 4 (MA: 934RJ9), Northbound in the area of 802 Walnut Street, Newton. Martinez stated as she was coming around the bend the sensor system for her vehicle activated and she attempted to pull her vehicle away from the double yellow line. Martinez stated the vehicle locked up and she made contact with MV2. Martinez reported no injuries on scene. I observed moderate damage to the front driver side area of MV1. The operator of MV2, Gerard Kenneally (S75917793), stated he operating his 1999 Ford E150 (MA: 3AL821), Southbound in the area of 802 Walnut Street, Newton. Kenneally stated MV2 was traveling Northbound on Walnut Street and crossed over the double yellow line and side swiped the driver side of his vehicle. Kenneally reported no injuries on scene. I observed moderate damage to the driver side of MV2 from the driver door to the rear bumper area. I spoke with a witness to the accident, Bryan Stewart, via telephone. Stewart stated he was traveling Southbound on Walnut Street behind MV2 at the time of the accident. Stewart stated he observed MV1 drive over the double yellow line and into their travel lane. Stewart stated 3/4 of the vehicle was in the Southbound lane while it was traveling Northbound. Stewart stated MV1 crashed into MV2. Stewart stated if MV2 was not in front of him then MV1 would have crashed into him. After speaking with all parties involved, Martinez was mailed Massachusetts Uniform Citation T1272998 for Chapter 89, Section 4A (Marked Lanes).

Motor Vehicle 1 (MV1) was traveling northbound on Centre St. at the intersection of Walnut St. when motor vehicle 2 (MV2), which was also traveling northbound on Centre St., attempted to go straight from the left turn only lane. MV2 then sideswiped MV1, running MV1 off the road and into the curb. The operator of MV1, Drew Brookins, stated that the operator of MV2 pulled over at first, but when traffic cleared, immediately got back into the car and left at a high rate of speed heading northbound on Centre St. Mr. Brookins attempted to follow MV2, and observed MV2 take a right onto Paul Street (eastbound), but was unable to continue to follow MV2, as MV1's front drivers side wheel was blown out as a result of colliding with the curb. MV1 sustained drivers side damage, as well as a blown out right drivers side tire. MV2 likely sustained moderated drivers side damage, and possibly damage to the rear bumper. Mr. Brookins was able to get the license plate of MV2 (MA Reg. 9LE764), which comes back to a female (Desiree Allman) out of Braintree. Mr. Brookins described the operator of MV2 as a white male with a thin build, dirty blond hair and scruffy beard. Dispatch contacted Braintree PD who was able to provide a telephone number that is believed to be Ms. Allman's. I attempted to contact Ms. Allman, however, the number went directly to voicemail. I left a message asking Ms. Allman to call the NPD so that we could find out who was operating her vehicle at the time of the crash. Traffic Bureau Update (Officer Gaudet): On August 6, 2019, I attempted to make contact with the registered owner of MV2, Desiree Allman (S85529796), with a negative result. A Hit and Run inquiry was mailed to the Allman. As of August 8, 2019, I have left several voice mails for Ms. Allman, she has not returned my calls. Ms. Allman had a hearing today in Hingham District Court. I contacted them to confirm we have her current contact information.

I left a VM with the owner of MV2 requesting a call back. MV1 sustained minor passenger side damage. It is unknown what damage was caused to MV2 and whether it would be to the truck or the trailer. Traffic Bureau update (Officer Gaudet): On September 4, 2019, I made contact with the owner of LPS Enterprises, Tony Lisa. Lisa confirmed MA TRN: C98747 is registered to his company. Lisa stated he will look into who was using the trailer on 8/30/19 and will contact me with their information. I spoke with the operator of MV2, John Subilosky (S89884850). Subilosky stated he was operating a 2011 GMC Sierra (MA CON: T58930) with MA TRL:C98747 attached to it on the afternoon of the accident. Subilosky stated he was not aware he was involved in an accident and stated he would have stopped had he known. Subilosky stated there was a bobcat in the trailer at the time of the crash. Subilosky stated there is no damage to the trailer. Due to the size and weight of the trailer attached to MV2, it is reasonable to believe if MV2 made minor contact with MV1, the operator of MV2 would not have been aware. Subilosky has been added to this report as the operator of MV2.

Orama stated the vehicle behind him attempted to pass him on his right and the front passenger side of MV2 crashed into the rear driver side of his vehicle. I observed minor damage to the rear driver side bumper area. Orama reported no injuries. The operator of MV2, Michael Ansel (S11557140), stated he was operating his 20003 Mercury Marquis ( MA: 992SK2) near 2000 Commonwealth Avenue (W). Ansel stated he was having an emergency and was trying to get around traffic to use a restroom at a local business. MV2's front passenger bumper crashed into MV2's rear driver bumper area. Ansel reported no injuries. Ansel stated he was responsible for the accident to me.

On Tuesday, September 10, 2019 while assigned to Traffic unit N525, I responded to the area of Beacon Street and Roslyn Road, Newton for a report of a motor vehicle/bicycle crash. The weather at the time of the crash was overcast. The road surface was dry. Beacon Street and Roslyn Road are both public ways maintained by the City of Newton. I spoke with the bicyclist involved in the crash, Steven Greenzang (S96615185). Greenzang stated he was traveling Westbound on Beacon Street in the shoulder lane on his black Specialized bicycle. Greenzang had a yellow reflective vest on and was wearing a helmet at this time. Greenzang stated a vehicle in the travel lane to his left drove by him and hit the mirror located on his left handlebar. I observed damage to the mirror. Greenzang report no injuries. I spoke with the operator of MV1, Sarah Bonfanti (S66811372). Bonfanti stated she was parked in front of 1411 Beacon Street waiting to pick up a co-worker that resides at that address. Bonfanti stated a bicycle traveling on the sidewalk to her right crashed into the passenger side door mirror of her 2008 Toyota Scion (MA: 681YH1). Bonfanti stated the bicyclist was acting "crazy" during her interaction with him and he was trying to take her picture. Bonfanti stated she got back into her vehicle and moved up the roadway to wait for police to arrive on scene. I canvassed the area where the accident occurred with Sgt. Dempsey. There was glass from the passenger side mirror of MV1 and Greenzang's bicycle in the main travel lane of the roadway and the shoulder lane where Greenzang stated he was traveling. We also observed a large tree and a telephone pole on the sidewalk next to the roadway where the accident is alleged to have occurred. After speaking with all parties involved, observing the location of the glass in the roadway from the crash, and the location of the tree and telephone pole on the sidewalk where he accident occurred; Greenzang would not have been able to operate his bicycle on the sidewalk and be close enough to the roadway to make contact with MV1. Prior to clearing the accident, I asked Bonfanti if she had any questions and if she wanted to report any injuries. Bonfanti stated she was hit by the bicyclist during her exchange with him and began grabbing her left leg. I asked Bonfanti why she did not inform the officers on scene that she was assaulted. Bonfanti stated she said something to our Dispatch when she called. I asked Bonfanti if her leg was injured. Bonfanti stated I was putting words in her mouth and that she was just scratching her leg. Bonfanti then stated she was hit in the right shoulder. I asked Bonfanti if she would like an ambulance to respond and evaluate her. Bonfanti declined and stated she was more concerned about her mirror. I advised Bonfanti if she wanted to pursue charges against Greenzang she could do so in Newton District Court. I listened to the 9-1-1 recording Bonfanti made in regards to this incident. Bonfanti stated a pedestrian busted out her window. Bonfanti then stated to our dispatcher that the bicyclist lost control of his bicycle and smashed into her car. During her exchange with the our dispatcher, I did not hear Bonfanti say she was physically assaulted by Greenzang.

On September 30, 2019 at approximately 16:56 I responded to a reported hit and run crash at 195 Sumner St. Dispatch gave out a description of the vehicle that fled, described as a red pickup truck with a possible partial plate of" MVNA". I, along with several other units checked the area for the vehicle with negative results. I responded to 195 Sumner St. and spoke with the operator of MV 1, who explained that he was traveling Northbound on Langley Rd onto Sumner St when a red pickup truck attempted to pass the RP from behind. Due to Sumner St being a narrow, one way street, the pickup truck cut back in front of MV1 and struck the front driver's side of MV1 with the rear passenger side of the pickup truck.The pickup truck proceeded down Sumner St. towards Centre St. without attempted to stop. The operator of MV1 did not know the make or model of the vehicle, but described it as a full size pickup truck that was red/maroon with a black tailgate with a large "fox racing" sticker on it. The description of the vehicle that fled was broadcasted to units canvassing the area, but was not found.

On 10/16/2019 I took a motor vehicle Hit and Run report at the front desk of NPD Headquarters. The owner of Vehicle 1 states that on 10/15/2019 at approximately 1600hrs Vehicle 1 was parked an unoccupied facing southbound in a parking space of the Elm St Municipal Parking Lot 100 Elm St. Vehicle 1 was then struck by Vehicle 2 who was pulling out of an adjacent parking spot. As a result of the impact Vehicle 1 sustained minor damage to the passenger side rear bumper. Vehicle 2 left the scene without leaving any identifiable information. No injuries were reported, no vehicles were towed. The owner of Vehicle 1 stated that he found a note on his windshield from an anonymous witness stating Vehicle 1 was struck by a gray Honda CRV MA Reg 665SW5. No other identifying information was included on the note. An RMV query of 665SW5 revealed a registered owner of Marylou Gair of Boston MA. I was unable to obtain any further contact information for Gair. At this time the operator of Vehicle 2 is unknown. This crash report will be forwarded to the Traffic Bureau for further investigation.

On Tuesday, October 29th, 2019 at approximately 1916 hours, I, Officer Newton and Officer Healy responded to a motor vehicle accident at Highland St and Hunter St. Upon arrival I spoke with the operator of vehicle 2. She stated she was on Washington St in the right lane taking a right on Highland St. She stated vehicle 1 was in the lane to her left and cut in front of her during the right turn and caused the vehicles to collide. She then followed vehicle 1 onto Hunter St. The operator of vehicle 1 stated he took a left off Cherry St onto Washington St and needed to take a quick right on Highland St. He stated he was not in the right lane and looked and thought it was clear to make a right turn. He then thought he hit the curb but saw vehicle 2 following him so he turned on to Hunter St. Neither operator claimed they had any injuries. Both vehicles were able to drive away from the scene without a tow.

Vehicle #1 was traveling north on Chestnut St. In the area of Chestnut St. and Oliver Rd. Vehicle #1 struck the curb on its passenger side. Vehicle #1 then crossed the double yellow lines crossing into the south bound travel lanes. Vehicle #1 then crashed into and over the curb and snow bank traveling completely off the road and into a utility pole on the sidewalk. Vehicle #1 was sustained heavy front end damage and was elevated off the roadway on top of a snowbank with a utility pole indented into its front end. The crash into the utility pole caused the pole to split in half. The operator of vehicle #1 was uninjured and refused medical attention. The operator of vehicle #1 was cited with MGL 90-24/K operating under the influence, MGL 90-24/E negligent operation of a motor vehicle, 90-24/C leaving the scene of property damage and MGL 89-4A marked lanes violation (citation #T2079135 & #T2079136). On 12/19/2019 while assigned to WNSQ I took a report at the front desk of NPD HQ for a past single motor vehicle cash. On 12/19/2019 Vehicle 1 was on Exeter St attempting to turn around in the driveway of 1536 Commonwealth Ave. While entering the driveway Vehicle 1 slid on a patch of ice and exited the roadway. The vehicle came to final rest in the front yard of 1526 Commonwealth Ave. No injuries were reported. No vehicles required a tow. Vehicle 1 had no visible damage. It should be noted that at the time of the crash the weather was clear with freezing temperatures. The area had just experienced snow and freezing rain precipitation on 12/17/2019. The operator of Vehicle 1 stated that after sliding into the front yard she was able to drive Vehicle 1 back on to Exeter St. The operator of Vehicle 1 stated that while she was exiting the front yard her tires were spinning and may have caused minor damage to the lawn. The operator of Vehicle 1 stated that she attempted to notify the homeowner of 1526 Commonwealth Ave but nobody appeared to be home. Officer Claflin N493 responded to 1526 Commonwealth Ave and tried to raise the homeowner with negative results. Officer Claflin observe tire tracks in the front yard but due to the presence of snow and ice it is unknown how much damage had been done. No phone number is listed for the homeowner of 1526 Commonwealth Ave as a result I will mail notification of this crash to the homeowner.

On February 8th, 2020 at approximately 09:54 hours I responded to the area of Washington St and Centre St for a reported hit & run MV crash. The R/P, called back stating she was now parked waiting for police on Park St near Elmwood Pk. On my arrival the operator of MV #1 stated she was driving E/B on Centre Ave after first exiting the Mass pike when vehicle #2 abruptly changed into her lane striking her passenger side front side tire. On 3/16/20 I was dispatched to 127 Commonwealth Ave for a report of a MV(#1) crash into a fire hydrant. The operator left the scene and was shortly after identified as Mr Bai Lin Wu. Wu called NPD to report the crash once he arrived home. Ofc Ferguson took photos of the knocked over hydrant. City Water Dept was notified of the damage. I responded to Wu's residence. Wu stated he was travelling on the Commonwealth Ave W/B carriage road. Wu stated an unknown MV travelling behind him was trying to pass him on the left. Wu stated he turned to the right to allow the unknown MV to pass. Wu ran off the road to the right and struck the hydrant in front of #127 Comm Ave. Wu further stated he left because he was trying to get medication home as soon as possible to his ill wife. Wu's vehicle sustained moderate damage to the passenger side front end. Wu issued citation for CO Ch19/Sec75 failure to use care turning (went off road).

On 5/14/2020 at 2000 hours motor vehicle accident occurred at the intersection of murray rd and webster st. Capt. Dowling was on a detail and had assisted both parties with exchanging information. MV1 was driving eastbound on Webster St and stopped and waited to turn left onto Murray Rd due to the detour. Mv1 was in her lane when MV2 was turning right onto Webster from Murray rd. MV2 took the turn too wide and ended up colliding with MV1. There was damage to both front ends but both cars were able to drive away safely. No injuries were reported.

On 05/15/2020, at 2100 hours, I responded to the area 399 Washington St. for a single vehicle accident. Upon arrival, I observed a red Toyota Highlander (MA Reg. 82F930) with heavy front end damage from colliding with and knocking down a cement light pole. The operator of the vehicle had fled the scene prior to our arrival. The registered owner is Orlin R. Aguilar. Inside the vehicle were 'Five Star Moving' logs indicating that Aguilar works at 95 Adams St. The route Aguilar would take home from his employer's address to his house in Chelsea would take him past the crash location. Chelsea PD was contacted to check on Aguilar's wellbeing. Ofc. Serrano of CPD located Aguilar in his apartment taking a shower. He appeared intoxicated with redness on his nose and face. Aguilar stated his car was stolen and he had no idea why it would be in Newton. Chelsea PD's wellbeing check was performed over a half hour after the accident and, according to GPS, Aguilar's home is 14 minutes from the crash location. Dispatch contacted Uber and Lyft to inquire if Aguilar was picked up in the area of the crash. There are also surveillance cameras outside of Honda Village across from the crash. Charges are pending further investigation of the ride-share info and surveillance footage. Pictures of the damage to the city pole and vehicle were emailed to IT. Tody's responded and towed the vehicle. Washington St. is a public way maintained by the City of Newton.

On May 21, 2020 at approximarely 15:53 hours I responded to 732 Beacon St for a report of a motor vehicle crash. Upon arrival I spoke with both motor vehicle operators. Both vehicles were traveling westbound on Beacon St. MV1 was in front of MV2 but was traveling towards the right side of the lane and MV2 attempted to pass MV1 on the left hand side as the road widened and turns into two marked lanes approximately 25 feet after past where the vehicles collided. The operator of MV1 said that he had his left turn signal on and saw MV2 approaching on the right hnd side. He attempted to move over switch into the left hand turn lane and struck the front passanger's side of MV2. The operator of MV2 said that she was passing MV1 as he was in the far right of the lane and did not observe a turn signal. As MV2 was passing, MV1 turned into her and struch the front passanger side with the driver's side of MV1.

The operator of MV1 (MA Reg: 6KC172) stated he was on the Centre St Bridge attempting to turn left onto Washington St. MV1 stated that he was in the 4th lane, which is a left turn or straight only lane. MV1 stated that when the light turned green he proceeded with his left turn and stayed in his lane. MV1 stated that as he was turning he was hit in the drivers side rear fender by MV2. The operator of MV2 stated she was in the third lane which she believed was a "straight arrow" lane. MV2 stated as the light turned green she began to drive straight ahead and collided with MV1 who was in the lane to her right taking a left turn. MV2 sustained minor damage to the front passengers side bumper. It should be noted that the accident was originally reported as a hit and run. MV1 called the police because MV2 failed to pull over. MV2 was stopped by Watertown PD and returned to the scene. MV2 stated that she did not stop her vehicle because she did not believe an accident occurred. The intersection has three left turn only lanes, one left or straight, and one straight only. MV1 was in the left or straight lane, MV2 was in the left turn only lane. Both parties involved reported no injuries.

On Monday, July 29th 2020 at approximately 0646 hours, I responded to Dedham @ Nahanton St for a Hit & Run Accident. Upon arrival, I spoke with the operator of vehicle # 1, Jeremy Stolmeier. His vehicle (Mass Reg # 3914YA 2015 GMC COLOR BLACK) was traveling East on Nahanton St toward the intersection of Dedham St in the straight lane. Vehicle # 2 (Mass Commercial #S37782 RED GMC SIERRA) was being operated in the right turn only lane on Nahanton St toward the intersection of Dedham St. Jeremy stated as both vehicles approached the intersection of Dedham @ Nahanton St, vehicle number 2 merged into his lane and collided with the front ride side of his vehicle. Upon collision, Jeremy pulled over on Dedham St to inspect his damage and exchange information for the insurance companies. Jeremy stated Vehicle # 2 did not attempt to pull over and continued driving down Dedham St until Jeremy lost sight of him. Newton PD Units were informed of the license plate of vehicle # 2 and searched the area with negative results. Jeremy was unable to make out the operator of vehicle # 2. Vehicle # 2 came back registered to Consiglia Construction. I was able to make contact with Andrea Adams, the Massachusetts HR Generalist for Consiglia Construction. She plans on investigating within the company who the operator of the vehicle was, and tracking down the vehicle so it can be inspected for damage. This investigation is ongoing. There were no injuries and no tows required. Traffic Bureau update (Officer Gaudet): On Tuesday, June 30, 2020, I was contacted by the operator of MV2, Mark Finnegan (S90305291). Finnegan stated he was traveling from Needham into Newton while MV1 was in the lane to his left. Finnegan stated as they approached an upcoming intersection, MV1 merged into his travel lane. Finnegan stated as MV1 entered his lane it's front passenger side made contact with his rear driver side. Finnegan stated he felt something, but wasn't sure he was involved in a crash. Finnegan stated he later observed light scrapes to his rear driver side. I asked Finnegan why he did not stop if he believed he may have been involved in an crash. Finnegan stated he did not want to pull over because the other driver was being aggressive. Finnegan stated he reported the incident to his employer Consiglia Construction (owner of MV2). Finnegan stated he was not injured. Finnegan was advised and his information was added to this report to reflect him as the operator of MV2.

On Saturday, August 1, 2020 at approximately 1810 hours I responded to a single motor vehicle crash. Upon arrival I saw the vehicle completely off the roadway and in the middle of a grassed area next to a large tree. The operator of MV1 stated he was driving westbound on Kenrick St and attempted to take a left turn on Waverly Ave when he lost control of his car and drove over the curb and hit part of the tree. EMS arrived on scene and operator of MV1 signed a refusal. There was major damage to the front of the car making it inoperable. The vehicle was towed by Todys. Todys had to cut branches of the city tree to get the vehicle out. Pictures of the vehicle and tree have been submitted to IT.

At the scene the operator of Vehicle #1 was extracted by Newton Fire and transported to Newton-Wellesley Hospital by Newton Medics. At the Hospital I spoke with the Operator of Vehicle #1. Operator states that she was driving home from a friends house and didn't remember any of the details of the crash. From my investigation, as well as the statement from the witness, it appears Vehicle #1 under steered while trying to make a left turn. The vehicle struck the curb, left the roadway and rolled onto its side. The driver was issued a citation for a Marked Lanes violation. The vehicle was towed from the scene by Tody's.

On 9/10/2020 I was working N491 when I responded to the area of Adams St. for a report of an MVA unknown injuries. Upon arrival I spoke with Hasibullah Wahab who stated he was traveling southbound on Adams St. when an unknown vehicle traveling northbound on Adams St. approached with "blinding" headlights. Wahab stated he "was unable to see due to the headlights and swerved" striking Veh 2. Wahab was evaluated by Newton medics and signed a patient refusal. I located the Owner of Veh 2 Tim White. White requested his vehicle be towed off the street due to the fact it was leaking fluids and he did not feel safe driving it. Both Vehicles were towed by Todys. Both parties were provided accident report numbers.

On Friday, September 18th, 2020 at approximately 1619 hours I, Officer Newton, was dispatched to Brookline St and Spaulding Ln(both are public ways in the City of Newton) for a report of a vehicle that crossed over Brookline St and struck a tree and drove off with major front end damage. The RP was able to give a description of the vehicle as a White Audi with the license plate beginning with 8X. The RP also stated the vehicle was going down Dedham St and possibly turned down Marvin Ln. Sgt M. Wade located the vehicle, White Audi MA Reg 8XVP10, with major front end damage, parked outside of 45 Indian Ridge Rd and made contact with the operator, John Bigda. It should be noted that the house is 2 houses from Marvin Ln. Upon arrival I spoke to John. John stated he was driving his vehicle and got into an accident but it was not a big deal because he was able to drive home. John was not able to explain the details of his crash and was very evasive as to exactly what happened, just that he got into an accident because somebody cut him off. John continued to tell me multiple times nothing bad happened and everything was fine. John was very unsteady on his feet and was stumbling around and almost fell several times while I was trying to speak with him. John also had slurred speech and a strong odor of an alcoholic beverage coming from his person that I was able to smell even though I was wearing a mask. At this time I asked John to take field sobriety tests to which he refused by saying "I'm not taking the test." The Medics arrived on scene to evaluate John. John was not able to answer their questions and the medics decided he should go to the hospital for an evaluation. However, John refused to go to the hospital. NFD arrived on scene and stated they saw tire marks and debris at the location of the crash. Ofc. Guarino went to that location to take pictures that will be added to this report. Due to the above facts and because John was almost falling over while speaking with us, he was placed under arrest for Operating Under the Influence of Liquor. John was handcuffed (D/L) and searched prior to being placed in cruiser 499. Cruiser 499 was searched before and after transport for contraband with negative results. John was booked and read his Miranda rights by Sgt M. Wade and Ofc Donovan processed him. John was handed citation T2014721 for 89/4/A Marked Lanes Violation (Driving over the double yellow line and leaving the roadway to strike the tree on Brookline St), 90/24/C Leaving the Scene of Property Damage (Hitting a city tree and causing damage), and 90/24/K (Operating Under the Influence of Liquor, 2nd Offense.. 1st offense was 11/18/2004 OUI docket 0443CR2527A CWOF ). The citation was placed with John's property. The vehicle was towed by Todys.

On 09/30/2020 at approximately 19:32hrs members of the Newton Police Patrol Bureau responded to a motor vehicle crash near the intersection of Washington St and Eddy St. After an initial investigation of the crash the Patrol Bureau secured the crash scene and called in the Newton Police Traffic Bureau to investigate. At approximately 19:45hrs Sgt. Devine, Ofc Wilson and I (Ofc Schlegel) from the NPD Traffic Bureau responded to the scene to investigate. Upon arrival to the scene I was met by Ofc Kane of the NPD Patrol Bureau. Ofc Kane stated that the crash was located in the driveway of 74 Eddy St and that it involved a Ford F350 pickup striking two parked cars. Ofc Kane stated that the operator identified as Kyle Butler had already been taken to Newton Wellesley Hospital by Cataldo EMS for injuries. . Upon approaching the crash scene I observed the following: The crash occurred at night time hours at approximately 19:32EST near the intersection of Washington St and Eddy St. Both Washington St and Eddy St are public ways in the City of Newton. The weather conditions at the time of the crash were a dark and clear with a temperature of approximately 60 degrees F. Washington St is a four lane undivided roadway (2 westbound travel lanes and 2 eastbound travel lanes). Washington St lighted roadway which has concrete light poles on both the westbound and eastbound sides. Washington St was under construction at the time and had a base coat of asphalt applied. At the time of the crash Washington St did not have any painted lane markings due to ongoing construction. Washington St is separated from the road sides on both the eastbound edge and the westbound edge with fixed granite curbing. The roadway also had several orange traffic barrels and traffic cones located edges of the eastbound and westbound lanes. Affixed to the light poles and the traffic barrels were temporary no parking signs for the ongoing road construction. At the time of the crash no road construction activities were taking place. No major debris or other obstructions were observed in the roadway. Washington St is a major roadway in the City of Newton and travels west to East across the entire City of Newton. This area of Washington St where the crash occurred is in between West Newton Square and Newtonville Square, this area has a posted 35 MPH speed limit. Both West Newton Square and Newtonville Square are densely populated with several businesses and residential areas. The section of Washington St where the crash occurred is also traveled frequently by pedestrian, bicycle and motor vehicle traffic. At the time the crash occurred 19:32hrs This area of Washington St is still a heavily travelled roadway. I observed Vehicle 1 described as a 2003 Ford F350 color white (MA Reg 5DT664) located in the driveway of 74 Eddy St. 74 Eddy St is a multi-family dwelling house located on the corner of Washington St and Eddy St. The driveway of 74 Eddy St is on the Washington St side. Vehicle was faced easterly across the driveway. I observed Vehicle 1to have extensive damage to the entire vehicle including but not limited to the enclosed cab area, cab roof and all windows, front end, rear end, along with both driver and passenger side. I also observed city of Newton owned concrete traffic light resting on top of the cab area of Vehicle 1. Upon further observation of Vehicle 1 it appears that both front tires and rims were flat, heavily damaged and had little to no tread left. Both front tires were worn down to the interior steel belt. I also observed that Vehicle 1 did not have a valid inspection sticker. Located to the immediate front of Vehicle 1 and resting on the front end was a white fence which had damage consistent with the impact of a motor vehicle. Immediately to the driver's side of Vehicle 1 was Vehicle 2 described as a 2007 gray Subaru Forester (MA Reg 45D610. Vehicle 2 was facing westerly across the driveway. Vehicle 2 had extensive damage to the driver's side front end, rear end, in addition to the entire driver and passenger sides. At this time Vehicle 2's drivers door was in contact with Vehicle 1's drivers door. Both drivers doors could not be opened.The damage to Vehicle 2 appeared to be consistent with impact from another motor vehicle. To immediate passenger side of the Vehicle 2 I observed Vehicle 3 described as a 2011 Toyota Rav4 color black (MA Reg 47XB99). Vehicle was facing in an easterly direction across the driveway. I observed the Vehicle 3 to have extensive damage to driver's side rear quarter panel, passenger side front quarter panel and front end. Also observed was damage to south east corner of the dwelling of 74 Eddy St. This area of the dwelling is located adjacent to the drive way. The damage appeared consistent with the impact caused by a motor vehicle. Also observed were large amounts of debris located in the area of the crash which included the sidewalk, yard and the driveway of 74 Eddy St. The debris included pieces of concrete which appeared to have been dislodged from the concrete light pole. A city of Newton no parking sign was lying in the yard of 74 Eddy St. This parking sign would have been attached to the concrete light pole due to ongoing road construction on Washington St. I also observed two tire marks on a curb stone located next the westbound lane of Washington St. These tire marks were in front of the residence of 74 Eddy St and were to the immediate left of a storm drain. The tire marks were consistent with being struck by a motor vehicle. In between the tire marks and just after the curb there was a hole in the ground with wires sticking out of it. This hole appears to be the location the concrete light pole was located prior to impact. Tire marks were located on Washington St traveling from the east bound lane easterly across the westbound lane and ending at the tire marks on the curb. The total length of the tire marks was approximately 190 feet. After conducting an on scene investigation it appears that Vehicle 1 from the start of the tire mark in eastbound travel lane of Washington St traveled 190 feet in the easterly direct across the westbound lane of Washington St and impacted the curb in front of 74 Eddy St. The tire marks had lateral striations almost the entire length indicating that the front tires were rolling and sliding with little to no braking prior to impact with the curb. After impact with the curb Vehicle 1 exited the roadway, struck a concrete light pole. Vehicle 1 then traveled over a sidewalk, across the yard of 74 Eddy St and into the driveway of 74 Eddy St. Located in the driveway of 74 Eddy St were Vehicle 2 and Vehicle 3. Vehicle 2 was parked facing toward the rear of 74 Eddy St. Vehicle 3 located to the passenger side of Vehicle 2 and was parked facing towards Washington St. When Vehicle 1 entered the driveway it struck Vehicle 2. The force of the impact of Vehicle 1 striking Vehicle 2 caused Vehicle 2 to strike Vehicle 3. The force of the impact of Vehicle 2 striking Vehicle 3 caused Vehicle 3 to strike the southeast corner of the dwelling located at 74 Elliot St. Vehicle 1 continued across the drive way and struck a white fence before coming to final rest. Vehicle traveled a approximately 55 feet from when it initially struck to the curb till final rest. No other injuries were reported on scene. All involved vehicles were towed from the scene by Tody's Towing. After the investigation the operator of Vehicle 1 Kyle Butler was issued Citations T2015882 and T2015883 for: Mgl 90/24/O Recklass Operation of a Motor Vehicle Mgl 89/4B/A Failure to Operate in the Right Lane Mgl 90/20 Failure to Submit a Motor Vehicle for Inspection Mgl 90/7 Defective Equipment (worn front tire treads) Mgl 90/13A Failure to wear a seatbelt An immediate threat form was also filed with the MA RMV. On October 4th, 2020 at approximately 10:43 hours while assigned to N491 I responded to a report of a two car MV crash in the area of #353 Craft St. On my arrival I located the two involved vehicles, 1st, a black toyota rav4 and a blue mercedes 350. Operator of the black toyota, identified as Charlie Rodriguez reports driving S/B on Craft St when vehicle #2 who was behind him tailgating pulled around his vehicle then stopped in front of him causing him to crash into the rear of his vehicle. Operator of the blue mercedes, identified as Mario Santi reports driving S/B on Craft St and the cars in front of him were going to slow. He further reported that vehicle #1 stopped in front of him so he went around vehicle #1. Vehicle #1 then drove into his rear after he passed him.

THE OPERATOR APPEARED TO BE HAVING SOME SORT OF MEDICAL EPISODE (SEIZURE LIKE) AFTER THE CRASH. IT IS UNKNOWN IF THE PARTY WAS HAVING THE MEDICAL EPISODE PRIOR TO THE CRASH. MV1 WAS TOWED WITH FRONT END/WHEEL DAMAGE. THE OPERATOR WAS TRANSPORTED TO THE HOSPITAL.

On Wednesday, October 21st 2020 at 0341 hours, Myself N-499 and Ofc Crowe N498 responded to 524 Parker St for a single vehicle accident into a city tree. Upon arrival, I identified Mass Vet #VT36122, 2016 Dodge Sedan, Color Black had crashed into Verizon Pole number 340/50A. The airbags had been deployed. The operator was no longer in the vehicle. The operator was sitting down on the ground nearby . He was identified as Joshua Stomberg. Stomberg admitted he had operated the vehicle. Stomberg stated he was driving south on Parker St toward the VA Hospital for his work shift. Stomberg stated he was tired and did not feel alert. Stomberg also stated there was a heavy fog that made the road difficult to see. Stomberg's vehicle veered off the side of the road and collided with the pole causing heavy front end damage. Stomberg did not show any signs of being impaired by alcohol. An inventory of Stomberg's vehicle showed no discovery of any alcohol. It should be noted that on my way to the accident scene, I observed a very heavy fog on Parker St that made it difficult to see the road in front of me. Stomberg suffered an injury to his back and was transported to the VA Hospital by Cataldo Ambulance. Tody's towed the vehicle. Stomberg was mailed MA Uniform Citation # T2013041 for the amount of $ 105 for M.G.L Ch 89/S4A Marked Lanes Violation. Pictures were taken of the accident and submitted to Newton IT Bureau.

Narrative:
On Tuesday, October 27th 2020 at 18:40 hours, Myself N-499 and Ofc Crowe N498 responded to 546 Parker St for a single vehicle accident into a city tree. Upon arrival, I identified Mass Reg # 3GDM90, 2015 LEXS RX350, Color Gray had crashed into a City Tree. The airbags had not been deployed. The operator was no longer in the vehicle upon my arrival. He was identified as registered owner Brian Mcneill. Mcneill admitted he had operated the vehicle. Mcneill stated he was driving North on Parker St toward RT 9 headed home to Wellesley. Mcneill stated he was on his phone texting and lost sight of the road. Mcneill's vehicle veered off the side of the road and side swiped a City of Newton Tree causing heavy right side damage and loss of the front right tire. Mcneill's wife came to the scene to pick him up and drive him home. The set of golf clubs in the trunk of the vehicle was documented in the MV Inventory Crash Form. Mcneill suffered no injuries and signed a patient refusal with the medics. Tody's towed the vehicle. Mcneill was given MA Uniform Citation # T2013042 for the amount of $ 210 for M.G.L Ch 89/S4A Marked Lanes Violation and M.G.L Ch. 90/S13B Texting While Driving Violation. Pictures were taken of the accident and submitted to Newton IT Bureau.

On October 28th, 2020 at approximately 08:13 hours while assigned to N491 I responded to the intersection of Newtonville Ave @ Lewis Terrace for a report of MV crash. On my arrival I located the two involved vehicles on the other side of the intersection on Lewis St. Vehicle #1 a grey 2016 Gray Mazda SUV, Ma reg 7vb971 operator identified as Katherine Anna Erler. She reported that she was stopped S/B on Lewis Terrace @ Newtonville Ave when vehicle #2 drove into the front of her vehicle as he turned left from Newtonville Ave. Vehicle #2 was a Red Grand Jeep Cherokee, Ma reg 1HKT75 operated by Paul Damore. He reported being on Newtonville Ave E/B attempting to turn left(N/B) onto Lewis Terrace when he got distracted by another vehicle coming from Lewis St and drove into vehicle #1. All streets mentioned in this crash are public ways maintained by the city. There were no injuries in this crash and neither vehicle needed to be towed.

On 11/10/2020 I responded to the area of 150 California St for a motor vehicle crash involving a City of Newton owned vehicle. California St is a public way in the City of Newton. Vehicle 1 described as a 2020 Kenworth T880 Truck owned by the City of Newton Forestry Division, was making a right turn into the driveway of 150 California St from the eastbound lane of California St. Vehicle 2 described as a 1994 Acura Integra was parked in the eastbound lane of California St just prior to the driveway of 150 California St. While making the turn the rear tire of Vehicle 1 made contact with the front driver's side fender of Vehicle 2. Vehicle 1 sustained little to no damage, Vehicle 2 sustained minor damage to the front driver's side fender. No injuries were reported on scene. No tows were required. Pictures were taken and were submitted to the IT Bureau.

Operator of MV1 states he just left Wegman's where he works the overnight shift and was traveling home to 223 River Street. Operator of MV1 states he was traveling westbound on Commonwealth Ave and was going to take a right hand turn onto Lowell Ave when he doesn't know what happened and struck the concrete street light pole dislodging it from its base where it laid across MV1 and part of the roadway. It should be noted while speaking with the operator of MV1 there was a slight language barrier and he couldn't elaborate much on the events leading up to the crash. MV1 sustained heavy front end damage and air bag deployment. Medics were requested for an evaluation and found no signs of fatigue, impairment or injuries and cleared with a signed patient refusal. Coviello Electric was notified for the street light pole and the Newton Fire Department capped off the exposed wires from the base and it was taped off with caution tape. Tody's towing responded and removed the vehicle from the roadway. The operator of MV1 insisted on walking home from the scene.

The owner of MV#1 had parked his vehicle at 10:03 in front of 1269 Centre St and returned to his vehicle at 12:00 to discover that it had been struck. Attached to the windshield of MV#1 was a handwritten note from a party that witnessed the accident. MV #1 sustained moderate damages to its front driver's side bumper. I spoke to the witness via telephone and he stated he was travelling southbound on Centre St directly behind MV#2 when he observed MV#2 sideswiped MV#1. The witness then observed the operator of MV#2 got out of his vehicle, looked at the damages to MV#1, retrieved a can of paint from a party standing on the sidewalk, got back into MV#2 and drove away. The witness stated MV#2 was a black colored "Dodge Ram" pick up truck bearing MA REG 2HLR81. The damages to MV#2 should be to its rear passenger side bumper. The witness described the operator of MV#2 as a white male in his 50's, 6' in height with a muscular build. A query of MV#2 showed that it is registered to a female party out of Weymouth MA. I attempted to contact the owner of MV#2 at phone #781 974-0307 and left a voicemail. I spoke to the Weymouth Police Dept and they will attempt to make contact with the owner of MV#2. I was able to speak to the owner of MV#2 and she stated her ex-boyfriend Richard Woodard has possession of MV#2 but does not speak to him often. She does not know Richard's personal information and provided me with his contact number of 617 631-0596. I spoke to Richard and he stated he was contacted by the owner of MV#2 and was already enroute to Police Headquarters. Richard arrived shortly after and he confirmed that he is the operator of MV#2 and was on Centre St retrieving paint earlier. Richard stated he was not aware he struck any other vehicles. I inspected MV#2 and did not observed any damages to its entire right side. I observed some minor scraping marks on the rear passenger side tire of MV#2. The rear passenger tire of MV#2 would be the most likely part of the vehicle that made contact with MV#1 and also consistent with the witness's statements. Based on the statements made to me by Richard and the fact that there were no visible damages to MV#2 besides its rear tire, it is possible that Richard was not aware he made contact with MV#1. All parties involved were provided with this accident report and advised to contact their insurance providers. There were no reported injuries to all parties involved. I will not pursue this matter further.

On 12/09/2020 I responded to the area of 849 Beacon St for a motor vehicle crash involving a City of Newton owned motor vehicle. Beacon St is a public way in the City of Newton. Vehicle 1 was located across from 849 Beacon St in a parking stall located in the eastbound lane of Beacon St. Vehicle 2 a City of Newton owned motor vehicle was parked in a parking stall directly in front of Vehicle 1. Vehicle 1 then struck Vehicle 2 while attempting to pull into the eastbound travel lane. As a result of the impact Vehicle 1 sustained minor damage to the passenger side front bumper, Vehicle 2 sustained minor damage to the drivers side rear quarter panel. No injuries were reported on scene. No vehicle tows were required. Pictures were taken of the scene and submitted to the NPD IT Bureau.

Witness Kaitlyn Coker stated she was travelling on Beacon St W/B directly behind a white sedan bearing MA reg 3910ZR(#2). Coker stated this vehicle while appearing to pull over to the right in an attempt to park, struck an unoccupied parked MV MA reg 2BAK94 (#1) in front of 845 Beacon on the driver's side front. After witnessing this Coker pulled over in front of the white sedan at which time the white sedan sped off leaving the area. Coker stated the white sedan operator appeared to be a white female with curly brown hair, no further description. Query of 3910ZR comes back registered to a white Lexus sedan owned by Mark C. Brown of 114 Monroe St Dedham MA 02026. An attempt to find a phone listing for this vehicle was negative. I left a note on windshield of #1 with witness and other vehicle insurance company listing. #2 would have damage on the passenger side with grey paint from #1. Traffic Bureau Update (Officer Gaudet): On Friday, December 18, 2020 I spoke with the owner of MV2, Mark Brown (S38802538). Mr. Brown stated he was operating his 2016 Lexus ES (MA: 3910ZR) on Beacon Street at the time of the crash. Mr. Brown stated he was thinking of parking in the area. Mr. Brown stated when he was pulling into the spot he decided he didn(cid:31)t have enough time to do his shopping and left the area. Mr. Brown stated he did not know he was involved in crash. Mr. Brown stated when he got home he saw a scratch on his passenger side but did not call the police because he thought insurance would handle it. Mr. Brown will be mailed Massachusetts Uniform Citation T0115919 for Newton City Ordincance Chapter 19, Section 75 (Failure to Use Care). Beacon Street is a public way maintained by the City of Newton.

On Saturday January 2, 2021 at approximately 0115 hours I was dispatched to 229 Auburn St for a motor vehicle accident into a tree. MV1 was operating eastbound on Auburn St when it crossed over the marked center lane. MV1 struck the westbound side curb and jumped onto the sidewalk. MV1 then continued on the sidewalk until it struck the tree in front of 229 Auburn St. MV1 sustained total front end damage, as well as driver side damage. The driver side damage was to both driver side doors and both driver side tires were flat. MV1 was towed by Todys. MV1 operator was injured but conscious. However MV1 operator was unable to speak due to possible concussion injuries. MV1 was transported by Newton medics to Newton-Wellesley Hospital. MV1 was issued MA Uniform Citation T2015088 for marked lanes and speeding over reasonable and proper rate, and it will be sent to the MV1 operator via mail.

On 1/21/21 I was working N493 when I responded to the area 450 Lexington St for a report of damage to city property from an MVA. Upon arrival I observed a parking sign that had been broken in half, Parking Meters that had been ripped from the ground and completely smashed, and 1 MA reg plate 8AH567. The plate came back to 398 Wolcott St. where I spoke to Dylan Crook. Crook stated he had been driving MA reg 8ah567 when he crashed and then went home. Crook stated he was distracted and reaching down to get something when he went off the road. Crook was given Ma unifrom citation T2080020 for 90/24/C Leaving the scene of an accident with property damage, which was explained to him. The city responded to clean up debris. Operator of MV1 stated he was stopped at the red light at Centre Street and Cabot Street facing southbound. MV2 was traveling Northbound, crossed into southbound lane of traffic and struck his vehicle, head on. After the collision MV1 spun out and rested in the southbound lane at Centre Street and Converse Ave. The operator of MV1 was transported to NWH for evaluation. After locating the operator who had walked away from the accident, the operator of MV2 stated he was traveling Northbound on Centre Street and struck MV1. After striking MV1, MV2 continued traveling and struck a stone wall, located at 709 Centre Street. Operator of MV2 stated he may have fallen asleep and would not go further into the how the accident occurred or what lead up to it. The operator and passenger both refused to see the medics, stating they were okay. I advised them multiple times it be best to be evaluated and they stated no. The operator of MV2 is also going to be charged with M.G.L. 90/24/A leaving the scene of personal injury and M.G.L. 90/24/C leaving the scene of property damage. MV1 and MV 2 sustained major damage to the front of the vehicles. Todys was notified and both vehicles were towed. Photos were taken of the property damage and submitted to IT to be added to the report.

On April 12th, 2021 at approximately 11:15 hours I responded to a report of a MV crash in the area of Centre St near Marathon Physical Therapy. On my arrival I located both involved vehicles on Centre Ave @ Centre St. Vehicle #1 was a White flat bed HINO Truck model325826. The operator was identified as Amanda Sprague. She reported being stopped on Centre Ave at Centre St waiting to make a right turn onto Centre St. According to her there was a funeral procession in the right turn only lane so she was waiting for it to pass before proceeding. The procession cleared vehicle#1 then proceeded to turn right when she struck vehicle #2. She further reported due to her height of her truck she never saw vehicle #2 on her right side. Vehicle #2 was a blue Prius station wagon, MA reg 9TM360 that sustained heavy rear quarter drivers side damage. The operator/owner identified as Binnie Estrada. She stated that she was stopped for the traffic light on Centre Ave at Centre St in the second lane. She indicated that she did see vehicle #2 with its right turn signal on but the truck was to her left in the third lane. She did wait at first to yield to the truck to turn right in front of her but when it didn't go she began to go straight ahead. The truck then began to make its right turn simultaneously which led to the crash. Operator #2 later said she never saw any funeral procession in the right turn only lane.

THE OPERATOR OF MV#2 STATED THAT MV#1 HE WAS TRAVELING SOUTH BOUND ON LEXINGTON ST WHEN MV#1 CAME ONTO THE WRONG SIDE OF THE ROAD, STRUCK HIM ON THE DRIVER SIDE, CAUSING HIM TO GO ONTO THE SIDEWALK AND TAKE DOWN A TRAFFIC LIGHT POLE. THERE WAS DAMAGE TO BOTH D/S, P/S, AND REAR OF MV#2. MV#3 HIT AND RAN FROM THE SCENE. NO ONE GOT A PLATE OR DESCRIPTION OTHER THEN POSSIBLY A PICK UP TRUCK. BOTH VEHICLES WERE TOWED BY TODY'S TOWING AND BOTH OPERATORS DECLINED MEDICAL ATTENTION. TRAFFIC OFFICER GAUDET TOOK PICTURES OF DAMAGE TO THE CITY PROPERTY. SEE INDEPENDENT WITNESS STATEMENT.

Operator of MV1 stated she was traveling Westbound on Washington Street passing Four Points Sheraton, approaching the rotary. She stated she was in the far left lane, making a slight left turn over the Mass pike bridge. As she was approaching the left, MV2 made two lane changes, toward her vehicle, cutting her off. As MV2 attempting to drive in front of her, MV2 struck the passenger side front bumper. Operator of MV1 stated, the Operator of MV2 then got out of her vehicle, began yelling at her and hit her passenger side window. Hand prints were visible on the window. I asked the Operator of MV2 what had happened and what led up to the accident. The operator of MV2 stated she didn't recall what had happened. She stated she was following MV1 from a previous accident that had just occurred in Watertown, Ma. There was visible damage to the rear driver side of MV2 vehicle. I asked the operator of MV2 how she got the damage and she stated she did not know. MV1 had minor damage to the front passenger side bumper. MV2 had minor damage to the rear driver side. All parties stated they were not injured and were able to operate their vehicles. Watertown PD arrived on scene and also took a report for the accident in Watertown. Report # 21-7242.

On Friday, May 7, 2021, while assigned to Traffic unit N525, I responded to the area of Colby Road for a report of a crash involving a City of Newton owned vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with Mr. Michael Sanford from the City of Newton Water Department. Mr. Sanford stated his City of Newton owned vehicle (2013 Ford F-250 (MA MVN: M88493) was parked unoccupied (S) in front of 497 Centre Street, Newton. Mr. Sanford stated his emergency lights were on while he was marking out a service. Mr. Sanford stated he then observed a white 2005 Ford Ecowagon (MA CON: L61558) travel past his vehicle South on Centre Street and make contact with his driver side door mirror. Mr. Sanford stated the vehicle continued down Centre Street and he followed it to Rochester Road where is came to a stop. I observed damage to the driver door mirror. I spoke with the operator of MV2, Ms. Jerrie Spencer (S12241050). Ms. Spencer stated she knew she made contact with MV1's driver door mirror, but was trying to make it to her appointment at the Faulkner Hospital and that is why she did not immediately pull over. I observed damage to the front passenger side door mirror. Ms. Spencer was provided with Massachusetts Uniform Citation 994181AA for Chapter 89, Section 4A (Marked Lanes Violation). Centre Street is a public way maintained by the City of Newton. Photos were taken of both vehicles and submitted to the IT Bureau.

MV1 was traveling Southbound while MV2 traveled Northbound on Lowell Ave. Around the intersection of Foster St at Lowell Ave, MV2 hit MV1's driver's side mirror with their driver's side mirror. MV1 side mirror was smashed and glass was inside the vehicle and the operator came in contact with some of the glass due to the driver's side window being open during the time of the collision. MV1 operator, John Mole, spoke with me at his residence after the accident because the operator of MV2 drove away from the scene without giving their information. MV1 operator had some dried blood on his left hand and left leg from the glass of the side mirror but refused medical treatment. He stated that after the crash, both MVs pulled to the side and operator of MV2 came out and said he didnt have his license and said he was in a hurry for an appointment and drove off. MV1 operator was able to read and write down MV2 license plate before it left the scene. MV1 operator describes MV2 operator as a tall 6'2" African male with short hair. MV2 was a company van belonging to MAB Community Services Inc. Traffic Bureau update (Officer Gaudet): I contacted MAB Community Services, Inc. to identify the operator of MV2. Heather Ripley, Director of Residential Services at MAB Community Services stated the operator of MV2 is Mr. Moses Mubiru. Ms. Ripley stated Mr. Mubiru provided them with a picture of the other vehicle involved in the crash and stated the other operator would not exchange information with him. On Thursday, May 27, 2021 I spoke with Mr. John Mole. Mr. Mole stated he was traveling South on Lowell Avenue towards Walker Street. Mr. Mole stated MV2 was traveling North on Lowell Avenue towards Washington Street. Mr. Mole stated as the vehicles passed each other their driver door mirrors collided. Mr. Mole stated both parties pulled over to assess the damage. Mr. Mole stated the operator of MV2 stated he did not have his license with him and it was a company vehicle so he did not have the registration. Mr. Mole stated the operator of MV2 stated, "I have to go I have an appointment, I have to pick someone up." Mr. Mole stated the operator of MV2 took a picture of his license plate and left the area without attempting to exchange information. I then contacted Mr. Moses Mubiru (S83293592). Mr. Mubiru acknowledged he was the operator of MV2 at the time of the crash. Mr. Mubiru stated he was operating MV2 South on Lowell Avenue towards Washington Street when his driver door mirror collided with the driver door mirror of MV1. Mr. Mubiru stated he did not exchange information with the operator of MV1 because that operator was unwilling to exchange information. Mr. Mubiru stated he took a picture of MV1's license plate and reported it to his employer. Mr. Mubiru's information was added to this report to reflect him as the operator of MV2.

Narrative:
Operator of MV#1 stated he was driving eastbound on Auburndale Ave when MV#2 came towards him. MV#2 swerved into the wrong lane and crashed into the driver side of MV#1. MV#1 attempted to pull up on the curb to avoid the crash and got a flat tire on the front passenger side. MV#1 sustained damage to the entire drivers side of the vehicle and had to be towed from the scene. MV#2 was described as a RAM pick up truck that fled the scene. I was able to view dash cam footage of the crash which confirmed the events as described. The truck did not have a front license plate and was not able to be identified. It sustained damage to the drivers side. Officers checked the surrounding area with negative results. David Donahue from Tody's towed the vehicle from the scene. The occupants were uninjured and took an Uber.

Narrative:
On 6/18/21 at approximately 0119 hrs Sgt. Fleming, Officer L. Keefe, and I, Officer Mildner responded to Sumner St @ Willow St. (public ways in Newton) for a report of a single motor vehicle crash. Upon arrival I noticed MA Reg: 8SP545 up on the sidewalk, air bags deployed, with heavy front end damage from what appeared to be from crashing into a tree. The operator was identified as Saurabh Korgaonkar, who was being evaluated by the medics and signed a patient refusal. I spoke with Korgaonkar and asked him what happened. As I began speaking to Korgaonkar I noticed he a strong odor of alcoholic beverage coming from his breath, glassy eyes, and slurred speech. Korgaonkar stated he was driving home from Brookline somewhere in the area of Commonwealth Ave. and Washington St. I asked Korgaonkar where he was specifically coming from and he stated "Abbey". I asked Korgaonkar if he had anything to drink while he was there to which he stated "two drinks". It should be noted Korgaonkar was unsteady on his feet and seemed extremely confused about what just happened. Given the damage from the crash, my conversation with Korgaonkar, and Korgaonkar's current state, I asked him if he was willing to perform Field Sobriety Tests. Korgaonkar stated he was willing to take the tests. I asked Korgaonkar if he wears glasses or contacts to which he stated "no". I asked Korgaonkar if he had any medical conditions that would affect his ability to take the tests to which he stated "no". I asked Korgaonkar if he felt comfortable in the shoes he was wearing to which he stated "yes". I brought Korgaonkar over to a clear spot on the side of the road on the fog line where the area was illuminated by the street light and cruiser spotlight. There were no blue lights on the cruisers. I used a field sobriety card I carry with me on my person to explain and instruct the tests. The first test I administered to Korgaonkar was Horizontal Gaze Nystagmus. I explained to Korgaonkar I was going to use a pen as the stimulus and to only follow it using his eyes only, not his head. Korgaonkar stated he understood the directions. The first attempt, Korgaonkar follow the pen using his head. I explained to Korgaonkar not to follow the pen using his head, but his eyes only. Korgaonkar stated he understood. The second attempt, Korgaonkar followed the pen using his head. I advised Korgaonkar again, to only use his eyes. On the third attempt, Korgaonkar followed the pen using his eyes only and I saw distinct nystagmus at maximum deviation and onset of nystagmus prior to 45 degrees. The second test I administered was the 9 Step Walk and Turn. I demonstrated the test for Korgaonkar and instructed him on how to complete the test. I placed my left foot on the fog line and right foot, heal to toe in front of the left. I demonstrated three heal to toe steps on the line, counting out loud, and keeping my hands by my side. Korgaonkar was advised to take 9 heal to toe steps, and then turn around and complete 9 more heal to toe steps. Korgaonkar stated he understood. During the test Korgaonkar did not place his heal to toe on steps 3,4,5. Korgaonkar almost fell over on the fourth step. Korgaonkar did not count out loud his steps and was using his arms for balance. On Korgaonkar's return, he did not count out loud again, almost fell to the side on step 6, was using his arms for balance, and stopped the test on his own at step 7. The third test I administered was the one leg stand. I explained to Korgaonkar the instructions and demonstrated the test for him. Korgaonkar stated he understood. I placed my hands by my side, feet together, raised my right foot six inches while looking at my foot, and counted 1001, 1002, 1003. I asked Korgaonkar if he understood, and not to stop counting until I told him to. Korgaonkar stated he understood. On the first attempt, I told Korgaonkar to start and he didn't lift a foot off the ground. On the second attempt, Korgaonkar lifted his left leg off the ground two inches, was looking straight ahead, and was not pointing his left foot. I explained to Korgaonkar his foot needed to be six inches off the ground, pointed, while keeping his eyes on his foot. On the third attempt, Korgaonkar dropped his left foot on 1004 and lost his balance on 1012. I concluded the test at 1020 counts. Based on my observations and Korgaonkar's performance on the field sobriety tests, I concluded that Korgaonkar had been operating a motor under the influence of alcohol. Korgaonkar was handcuffed (DL), searched, and placed in the rear of 496 where he was seat belted. The prisoner compartment was checked prior to and after transportation to NPD. Korgaonkar was booked by Sgt. Fleming and processed by Officer Ka. Donahue in the normal fashion. During booking, Korgaonkar was read his rights and asked if he wanted to take a breathalyzer test. Korgaonkar refused the breathalyzer test. Korgaonkar was notified his license was suspended, and his MA drivers licensed seized. Officer L. Keefe stood by and waited for Tody's to arrive and tow away the vehicle. A towed MV form was filled out and placed in appropriate mailbox. Korgaonkar was charged with the following: MGL Ch. 90 Sec. 24/J OUI Alcohol (Korgaonkar crashed his vehicle and failed field sobriety tests) MGL Ch. 90 Sec. 24/E Negligent Operation of MV (Korgaonkar failed to operate his vehicle in a safe manner) MGL Ch. 89 Sec. 4/A Marked Lanes Violation (Korgaonkar crossed the fog line, went onto the sidewalk, and hit a tree) Photo's of scene sent to IT. A court packet was filled out with all pertinent information and placed in the court prosecutors mailbox.

On Wednesday, June 23, 2021 while assigned to Traffic unit N525, I responded to St. James Street at the Washington Street rotary for a motor vehicle crash involving a Fallon Ambulance and a City of Newton owned vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. St. James Street and Washington Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Mr. Michael Cannistraro. Mr. Cannistraro works for the City of Newton Parks and Recreation Division. Mr. Cannistraro stated he parked his City of Newton owned 2019 Ford F-150 (MA MVN: M3116A) to the right side of St. James Street (W) at the Washington Street rotary. Mr. Cannistraro stated he exited his vehicle and was trimming branches hanging over the sidewalk of this area when he observed a Fallon Ambulance Company owned van drive Westbound on St. James Street. This vehicle's front passenger side door mirror made contact with the front driver side door mirror of his unoccupied vehicle and left the scene with out stopping and exchanging information. Newton Dispatch contacted Fallon to attempt to located the driver with a negative result. The passenger side door mirror for the Fallon vehicle was observed resting on the roadway next to MV1. Pictures were taken of the damage to MV1 and submitted to the IT Bureau. I spoke with Chris O'Malley from Fallon who stated he will look into what vehicles were in the area at the time of the crash and get back to me. On June 24, 2021, I followed up with Fallon and spoke with the on duty supervisor for Fallon, Mr. Richard Souza. Mr. Souza stated he looked into the incident on his end and no one has reported being involved in a crash or reported any damage to their vehicle. If any information becomes available Fallon will update me.

On Thursday 6/24/2021 at 5:40 pm I responded to MV accident on Derby St at Sheridan St. Upon my arrival I spoke to the Operator of MV 1 (MA REG 48VP86) and she stated that she was traveling westbound on Derby St and tried to take a left on to Sheridan St when MV 2 (MA REG 3WVD91) tried to pass her on the left side by going on the wrong side of the road and collided with her car. MV1 had damage on the driver side front wheel well. I spoke with the operator of MV 2 and he stated that he tried to pass MV 1 on the left because he thought she was trying to pull into a driveway. MV 2 had damage on the passanger side. Both vehicles were able to be driven away and neither party was injured.

On 7/27/2021 at approx 0939hrs while assigned to 497 I responded to the intersection of Fenwick Rd and Tamworth Rd for a report of a two car crash with possible injuries. Upon arrival I observed Ma Reg 1CNR59 being operated by Svertlana Savenco pinned between the southern curb of Tamworth facing Eastbound and Ma Reg 62EZ69 being operated by Renata Pawska. I spoke with Renata who stated she had been travelling NB on Fenwick Rd intending to turn right onto Tamworth when she saw Tamworth east of Fenwick was shut down due to a detour, at that time Renata turned left colliding with Svetlana. I spoke with Svetlana who stated she was travelling straight ahead when Renata turned into her. Both vehicles towed by Todys, NFD put down speedy dry for fluids, Renata signed a refusal with Fallon Ambulance, Svetlana was transported by Fallon complaining of leg and back pain.

Narrative:
On Wednesday, August 4, 2021, while assigned to marked unit N493, I responded to the area 2115 Commonwealth Avenue, Newton with several other units for a report of a motor vehicle crash with entrapment. Commonwealth Avenue is a public way maintained by the City of Newton. The weather at the time of the crash was clear and sunny, The road surface was dry. On arrival, I spoke with the operator of MV1, Mr. Robert Bemis (S57110680). Mr. Bemis stated he was operating his 2019 Nissan Murano (MA: 1WHT71) on Commonwealth Avenue (W) towards Melrose Street. Mr. Bemis stated a red SUV traveling on Commonwealth Avenue (E) entered his travel lane and he had to swerve to avoid crashing head on. The front driver side of MV2 crashed into the driver side of MV1. I observed moderate damage to the entire length of MV1's driver side. Mr. Bemis reported no injuries and signed a patient refusal with Fallon Medics. Mr. Bemis was able to operate his vehicle away from the area. I spoke with the operator of MV2, Mr. Robert Shea (S86188377). Mr. Shea stated he was operating his 2011 Chrysler Town and Country (MA: 2LYT84) on Commonwealth Avenue (E) towards Lexington Street. Mr. Shea stated he was attempting to make left turn towards Lexington Street when MV1 hit him. Mr. Shea was transported to Newton Wellesely Hospital for evaluation. I observed signifcant damage to the front driver side of MV2. MV2 was removed from the roadway by Tody's Towing. The point of impact of the crash occured in the Westbound lane of Commonwealth Avenue. While traveling Eastbound on Commonwealth Avenue, MV2 crossed the double yellow unprotected median on the roadway and entered the Westbound travel lane. MV2 applied his brakes leaving skid marks to the point of impact and crashed into the driver side of MV1. The operator of MV2 will be cited with Massachusetts Uniform Citation 090421AB for Chapter 89, Section 4a (Marked Lanes Violation).

On Sunday, August 22, 2021, I was contacted by Sgt. M. Wade of the Newton Police Traffic Bureau at 0032 hours. Sgt. Wade stated a serious motor vehicle crash occurred on Washington Street in Newton and it involved a City of Newton marked police cruiser (N494). Sgt. Wade stated Officer Katelyn Pohlman and the operator of the other vehicle involved were transported to the hospital with potential serious injuries. I was requested by Sgt. Wade to respond to the scene from my residence and investigate the crash further. On arrival at the scene, I observed the two vehicles involved in the crash to be a 2015 Ford Explorer (MA: 586RT7) and Newton Police marked unit N494 (MA MVN: MP494B). These vehicles were observed disabled on the roadway in front of 612 Washington Street, Newton. There was a large debris field comprised of various parts from both vehicles on the roadway and in the parking lot of 612 Washington Street. The road surface at the time of my arrival was wet and there was light rain falling. Sgt. Wade reported that at the time of the crash the road was completely dry and it had not started to rain yet. Washington Street is a public way maintained by the City of Newton. The portion of Washington Street where the crash took place supports two- way travel, Eastbound and Westbound. There are two travel lanes in both directions. Travel direction is divided by a double yellow line in the middle of the roadway. The area is well lit and features numerous light poles illuminating the roadway. The closest intersection to this location is Crafts Street just west of the crash location. MV1 (MA: 586RT7) had substantial front-end damage, especially to its front passenger side and was inoperable. I observed this vehicle at its final rest on the roadway with its front end facing Northbound on Washington Street just over the double yellow line in the middle of the roadway. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV1 was removed from the roadway by Tody's Towing. MV2 (MA MVN: MP494B) had substantial front end damage and was inoperable. The impact from the collision caused the vehicle to spin uncontrolled and the final rest, with the front end of the vehicle facing the front entrance to the 7-11 (S). The vehicle's final rest was on the sidewalk in front of 612 Washington Street. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV2 was removed from the roadway by Tody's Towing. The Officer involved in the crash is Officer Katelyn Pohlman. Officer Pohlman stated she was operating Newton Police marked unit N494 on Washington Street (E) past Crafts Street. Officer Pohlman stated as she was operating her vehicle past 612 Washington Street (7-11), a vehicle in the westbound lane of Washington Street abruptly crossed the double yellow line in the roadway and entered her travel lane head-on. Officer Pohlman stated she did not have an opportunity to react to the vehicle in front of her and the front passenger side of MV1 crashed into the front end of her vehicle (MV2). Officer Pohlman sustained injuries to her left forehead area and both of her arms. Officer Pohlman was later transported to Brigham and Women's Hospital for evaluation and was released a short time later. It was reported to me by numerous Officers on scene that Officer Pohlman exited her cruiser after the crash and rather than tend to her own injuries, she focused her efforts on rendering aid to the operator of the other vehicle involved. Officer Pohlman found the operator of the other vehicle lying on the floor of the front passenger seat under the dash. Officer Pohlman rendered aid to the operator while waiting for Newton Medics to arrive. Officer Pohlman was also assisted in rendering aid to the other operator by Officer Conary and Officer Larson. Officer Larson reported that while interacting with the operator of MV1, the operator repeated numerous times, "Is she ok? Is she ok?". Officer Larson also stated the operator said to him "I feel bad because I was texting on my phone". Officer Larson stated the operator of MV1 works for the Department of Corrections Special Operations Unit and has Department issues equipment inside the vehicle. The following items were placed into Support Services, locker 9 for safekeeping by Officer Larson: 1 Gray Pelican Gun Case, 1 Black Gun Belt, 1 Taser, 1 Can OC Spray, 15 (50) Round Cases Of Ammunition. The operator of MV1 (MA: 586RT7) was identified as Mr. David Cafarelli (S16965940). Mr. Cafarelli was transported to Newton Wellesley Hospital for a potential head injury before my arrival. I spoke with Mr. Cafarelli at Newton Wellesley Hospital in the Emergency Room with Sgt. Wade. Mr. Cafarelli's mother and father were present when we spoke with him. Mr. Cafarelli stated he was out with friends before the crash and was driving to the 7-11 on Washington Street. Mr. Cafarelli admitted that before the crash, he had his cell phone in his hand while he was operating his vehicle. Mr. Cafarelli stated he does not remember if he was texting or changing the music on his phone at the time of the crash. Mr. Cafarelli stated he looked up and was unable to avoid the vehicle (N494) directly in front of him. Mr. Cafarelli stated he also does not believe he applied his brakes before the collision. When we spoke with Mr. Cafarelli in the Emergency Room, he was still waiting for test results to determine the severity of his injuries. On Monday, August 23, 2021, Sgt. Wade responded to 612 Washington Street, Newton to retrieve surveillance footage from the 7-11 of the crash. 7-11 stated their outside surveillance system was not operating correctly, but they were able to capture some of the incident from a camera inside the store. I observed this footage. MV1 is observed traveling on Washington Street (W) in the westbound travel lane in front of the storefront. MV1's driver side is then observed traveling over the double yellow line in the middle of the roadway and into the eastbound lane of Washington Street. A head on collision ensues and MV1 can be seen rotating counterclockwise in the roadway backwards to its final rest location on Washington Street. MV2 is then observed at its final rest position on the sidewalk facing southbound in front of the 7-11 storefront. After completing my investigation, I have surmised the following. Mr. Cafarelli was operating his 2015 Ford Explorer (MA: 586RT7) in the area of 612 Washington Street, Newton. Mr. Cafarelli was holding his cell phone in his hand while operating his vehicle. Mr. Cafarelli became distracted while he was operating his vehicle due to his attention being focused on his cell phone. Mr. Cafarelli traveled over the double yellow line from the westbound lane of Washington Street and entered the eastbound lane of Washington Street. Newton Police marked unit N494 was traveling on Washington Street (E) in the area of 612 Washington Street at this time. Mr. Cafarelli's vehicle crashed into the front end of Newton Police marked unit N494. Mr. Cafarelli was not wearing a seatbelt and as a result of the impact from the crash, he was ejected from the driver seat and his final rest inside the vehicle was in the flood of the front passenger seat. As a result of my investigation, Mr. Cafarelli was issued Massachusetts Uniform Citation for Chapter 90, Section 13B (Hands-Free Violation), Chapter 89, Section 4a (Marked Lanes Violation), and Chapter 90, Section 13a (Seat Belt Violation). Photos from the crash scene and the surveillance media from 7-11 capturing the crash were submitted to the IT Bureau.

Vehicle 1 was travelling northbound, behind vehicle 2 on Centre St. Vehicle 2 went around vehicle 1, in order to get into the left hand lane, which started up ahead. Vehicle 2 changed into the left hand lane with no blinker, while vehicle 1 was already in it. I observed moderate damage to the front drivers side of vehicle 2, and minor damage to the passenger side of vehicle 1. Newton medics evaluated all parties, and cleared with refusals. Todys towed vehicle 1 because of the fact that it was unregistered. A Massachusetts uniform citation (T2080110), was issued in hand to the operator of vehicle 1. See incident report #21039072, for criminal charges. Witness 1 was travelling behind vehicle 2, and has the accident on video. He was advised to email this to me, which I will have attached to the accident report.

On 09/15/2021 I was dispatched to a possible MVA hit and run involving an unmarked Newton Police cruiser. MV1, a Ryder rental truck, was driving southbound on Washburn St in the area of Pearl St. As MV1 turned left on Pearl St, the rear of the truck swung out, and struck MV2 (the unmarked cruiser) on the front drivers side by the wheel well and bumper. The accident was heard by the officer working the detail, and witnessed by the work crew. MV1 continued on Pearl St, and turned right on to Peabody St. He was followed by the unmarked unit which did not have emergency lights and sirens. He obeyed all traffic laws, until he was pulled over by a marked police unit. Operator of MV1 stated he did not realize the truck had struck any vehicles on Washburn St. MV1 had minor damage to the passenger side rear. MV2 had damage to the front drivers side and bumper. Both vehicles were able to be driven from the scene. No injuries occurred. It appears the operator of MV1 would not have known the accident occurred until he was pulled over.

The operator of MV#1 stated she was travelling northbound on Centre St in the right side travel lane at the intersection of Beacon St when MV#2 merged into her lane from the left side travel lane striking her vehicle. MV#1 sustained minor damages to its driver's side front and rear doors. There were no reported injuries to the operator of MV#1 or its passenger. The operator of MV#2 stated he was travelling northbound on Centre St in the left side travel lane when the vehicle in front of him attempted to make a left turn onto Beacon St. The operator of MV#2 stated he attempted to go around the vehicle in front of him by merging into the right side travel lane when MV#1 came at a high rate of speed and made contact with his vehicle. MV#2 sustained minor damages to its entire passenger side. There were no reported injuries to the operator of MV#2. Newton Fire evaluated all parties involved and determined there were no injuries. The witness stated she was standing on Centre St at the intersection of Beacon St and observed the entire accident. The witness stated MV#1 was travelling straight on Centre St in the right side travel lane when it was struck by MV#2. The witness stated MV#2 was on Centre St in the left side travel lane directly behind a vehicle that was attempting to make a left turn. The witness stated MV#2 attempted to go around the vehicle in front it by merging into the right side travel lane striking MV#1.

On September 17th, 2021 at approximately 10:54 hours while working N491 I responded to a MV crash at the intersection of Harvard St @ Newtonville Ave. On my arrival I located both involved vehicles. Vehicle #1 was a 2019 White Ford F150 PU, Ma. commercial W26209, operated by Nathan Joseph. He stated he was stopped for the red light on Harvard St @ Newtonville Ave. The light turned green he proceeded to go forward when vehicle #2 came around him from his rear and turned right in front of him towards Newtonville Ave causing the crash. Vehicle #2 was a Grey 2016 JEEP Cherokee NJ plate R86GTJ, operated by a Jake Schneider. He reported going N/B on Harvard St approaching the intersection of Newtonville Ave when he went around vehicle #1 who he thought was parked. As he began to turn right E/B onto Newtonville Ave vehicle #1 drove straight into his vehicle. There were no injuries and no tows.

Both MV1 and MV2 were traveling eastbound Exiting Mass Pike Rt 90. MV1 proceeded into the lane of traffic on Washington St. MV2 Proceeded on the inside lane of MV1 in the exit ramp travel lane. MV1 then veered back into the exit ramp travel lane. MV2 struck the back right bumper of MV1 and MV1 caused damage to MV2's front driver side wheel well. MV2 was transporting a patient in the ambulance. All parties did not have any injuries due to the motor vehicle accident. MV2 transferred the patient into a new ambulance and parked MV2 in the parking lot of Newton Corner for their tow company to inspect.

On September 9, 2021 at approximately 10:56 hours I,Officer Guarino, responded to 79 Florence St for a report of a single vehicle crash into a gate and pole. Upon arrival, I observed MV1 with heavy front end damage crashed against a brick structure that holds the iron gate of the property. The gate was detached from the structure and on the sidewalk. The operator of MV1 told me that she was driving northbound towards Florence St when she observed a UPS truck entering the property. Operator of MV1 thought the UPS truck entered her lane of travel and swerved to the right which caused her to crash into the gate and brick structure. The operator of the UPS truck was on scene and had no damage on his vehicle. Multiple witnesses observed the crash who said that the UPS truck was traveling slow and stayed within its lane of travel. A witness said that the operator of MV1 was "flying" through the parking lot and swerved into the gate and structure. I observed the video footage at 79 Florence St where you can see MV1 traveling through the parking lot and it appeared that the UPS truck had already entered the property when MV1 took the corner and MV1 drove into the gate and Brick structure. I emailed the property manager to have the footage emailed to me, so I can attach it to this report. Witnesses, Rich (781)351-0172 and Mario (615) 981-4890 left their information with the UPS driver but I was unable to make contact. The witness I spoke with on scene wanted to remain anonymous.

On October16th, 2021 at approximately 10:13 hours while working N491 I responded to a MV crash in the area of Washington St @ Channing St. On my arrival I located both involved vehicles at the gas station at the corner of Washington St @ Thornton St. Vehicle #1 was a 2018 blue BMW X2, CT REG. AS41177 operated by a Narendra Roy. He stated he was going W/B on Washington St in the far right lane when vehicle #2 entered his lane causing the crash. Vehicle #2 was a blue 2018 Lexus LX570 MA plate HAYATI, operated by a Faten Akar. She stated she was travelling W/B on Washington St in the middle lane when she attempted to change lanes so as to get to a business at #341 Washington St when vehicle #1 struck her vehicle. She further stated prior to the crash she was already half in the other lane when vehicle #1 came from her rear and never saw him.

On November 9th, 2021 at approximately 13:58 hours while assigned to N491 I responded to a report of a MV crash involving a possible hit & run in the area of #300 Washington St . On my arrival I located vehicle #1 a 2021 Grey Toyota RAV4, MA plate 9143, operated by a Peter Dorsey. He reported being N/B on the Centre St bridge attempting to turn left onto Washington St W/B when vehicle #2 who was on his left went straight ahead(from a left turn only lane) striking his vehicle(drivers side ) as he was making his turn. Vehicle #2 was in the wrong lane for the direction the vehicle went. Mr Dorsey described vehicle #2 was an older model Grey Van, possible Ma. Reg.215N50, operated by a white middle aged female. She continued straight after the crash towards Watertown. The female operator did make eye contact with Mr Dorsey but never stopped after the crash. Watertown PD was notified with a BOLO for that vehicles description. Vehicle #2 left scene. Plate given by operator #1 came back to a different vehicle not matching vehicle #2's description.

OWNER STATES SOMETIME BETWEEN 1030-1230HR. SOMEONE HIT HIS MOTOR VEHICLE WHILE IT WAS PARKED IN FRONT OF 72 HIGHLAND ST. THE ACCIDENT CAUSED DAMAGE TO THE OWNERS LEFT REAR VIEW MIRROR. THE GLASS OF THE MIRROR REMAINED INTACT BUT THE MIRROR CAME UNHINGED. OWNER STATES THERE WAS NO WITNESSES TO THE ACCIDENT. THE OWNER OF 72 HIGHLAND TOLD THE OWNER OF VEHICLE #1 HE WOULD REVIEW HIS VIDEO FOOTAGE TO SEE IF HE HAS FOOTAGE OF THE ACCIDENT AND WOULD UPDATE IF ANYTHING WAS CAPTURED. OWNER REPORTS NO INJURIES AND WAS ADVISED TO CONTACT HIS INSURANCE COMPANY. CLEARED WITHOUT FURTHER INCIDENT.

On Tuesday, November 23, 2021, while assigned to Traffic unit N525, I was asked to investigate a past hit and run crash involving a City of Newton School Traffic Supervisor. Mr. Mark Dumoulin reported to the Newton Police Traffic Bureau that sometime during his assigned school crossing post on the morning of November 23, 2021, a passing vehicle made contact with his left leg while he was crossing children on Ward Street. I responded to Newton Wellesley Hospital to speak with Mr. Dumoulin. Mr. Dumoulin stated this morning, he was at his assigned school crossing post of Ward Street and Waverly Avenue in his official capacity as a City of Newton School Traffic Supervisor. Mr. Dumoulin stated he was wearing his department issued yellow reflective jacket and his department issued yellow hi-vis gloves. Mr. Dumoulin stated he also had his department issued stop sign with him while performing his duties. Mr. Dumoulin stated at some point during his post (0745-0830 hours), he was attempting to cross two children on Ward Street (S to N). Mr. Dumoulin stated he entered the crosswalk of Ward Street on the NE corner of Ward Street at Waverly Avenue. Mr. Dumoulin stated the pedestrian signal was activated at this time and the intersection traffic signals were all red. Mr. Dumoulin stated while entering the crosswalk, he observed a black Honda sedan traveling on Stuart Road (E) towards the intersection of Ward Street and Waverly Avenue. Mr. Dumoulin stated the license plate had 3 characters on it and believes the first letter may have been "E". Mr. Dumoulin stated the operator was a heavy set female with blonde hair in her 30's or 40's. Mr. Dumoulin stated the operator appeared to speed up towards his location to beat the light turning red. Mr. Dumoulin stated the vehicle then continued through the intersection onto Hammond Street (E), but it did not keep right with the normal flow of traffic and entered the Westbound lane of Hammond Street. Mr. Dumoulin stated he attempted to get the operators attention to stop her from continuing in the wrong lane of Hammond Street. Mr. Dumoulin stated at this time, the vehicle made contact with his left knee area and continued traveling down Hammond Street with out stopping. Mr. Dumoulin stated the children he was crossing were not near the vehicle and they did not see the contact. I asked Mr. Dumoulin if there were any other witnesses in the area. Mr. Dumoulin stated he does not believe there were. Mr. Dumoulin reported this collision to the Newton Police Traffic Bureau at approximately 1030 hours. I asked Mr. Dumoulin why he did not contact us when the incident first took place. Mr. Dumoulin stated he thought his injury was superficial and that he knew we were busy. At this time, Mr. Dumoulin is reporting a left knee injury as a result of the crash. Mr. Dumoulin stated vehicles travel on the wrong side of the medium on Hammond Street all the time and estimates it happens 3 times every school post. Mr. Dumoulin also stated the keep right sign on Hammond Street near the crash site needs to be addressed because some of the letters are peeling. I canvassed the area where the crash occurred for the fleeing vehicle with a negative result. I also covered the afternoon school post (1450-1535 hours) to ask passing students if they witnessed the crash with a negative result. I observed the "Keep Right" sign Mr. Dumoulin made mention of at this location. The sign appears to be in working condition and visible from Stuart Road. I took photos of the intersection where the crash happened and submitted them to the IT Bureau. On November 24, 2021, I covered Mr. Duboulin's school crossing post again for the morning post. While canvassing the area I observed a dark colored Mercedez sedan with a 3 character registration parked on Exmoor Street that resembled the vehicle Mr. Duboulin described as the fleeing vehicle. I attempted to contact the registered owner of this vehicle, Ms. Judtich Zukor. Ms. Zukor stated she could not speak at this moment and asked me to call her back. I will follow up with Ms. Zukor.

Upon arrival, vehicle 1 was on the median facing Northbound on Beacon St at the intersection of Beacon St and Washington St. There was damage to the front drivers side bumper. Vehicle 2 was facing Southbound on Beacon St at the intersection of Beacon St and Washington St. There was heavy damage to the front bumper of Vehicle 2 and it was leaking fluid. Operator 1 stated through a translation device on his cell phone," Look, I was coming slowly when I was going down the curve and the car lost the brake, I didn't have it." When queried through dispatch, it came back that operator 1 was unlicensed. Operator 2 stated she was traveling on Washington St and taking a left turn to head Southbound on Beacon St when vehicle 1 drove over the median and crashed into her head on. Both vehicles were towed by Tody's towing. It appeared that the brakes on vehicle 1 were functioning properly when Tody's towing moved the vehicle. Operator 1 and 2, both signed patient refusals with Fallon Medics. Operator 1 was handed Massachusetts Uniform Citation#T1445614 in hand for Unlicensed Operation (Incident report# 21049086).

On November 29th 2021, at approximately 1721hrs, Lizabeth Mansfield (operator 1), reported to the front desk of Newton Police Headquarters to report a past hit and run that occured approximately at 1711hrs. The accident occured at the intersection of Lowell Ave and Washington St. Operator 1 stated that she was driving Northbound on Lowell ave in the left turn only lane just prior to the intersection of Washington St. As she was driving, vehicle 2 was in the right lane traveling Northbound and their attached trailer side swiped the front bumper of vehicle 1. Operator 2 did not stop after the collision and continued onto Washington St. Operator 1 described vehicle 2 as a red landscaping truck with a trailer attached to it. She could not provide any more details or a plate of vehicle 2. There was heavy damage to front bumper of vehicle 1, but the vehicle was operable. It is unknown if there is any damage to vehicle 2. Operator 1 stated she did not need medical attention.

Narrative:
Witnesses at the scene informed me that MV#1 was traveling west on Commonwealth Ave in the area of #2102 when MV#2, who was traveling east on Commonwealth Ave in the same area crossed the yellow line and crashed into MV#1. The operator of MV#1 was upset and sitting in MV#1 when I interviewed her and only stated that her left hip hurt. Operator #1 refused medical treatment at that time and wanted to wait for her mother to arrive before making any decisions. The operator of MV#2 stated that he was traveling east on Commonwealth Ave in the area of #2102 in the left hand lane when a pedestrian appeared in the roadway. According to operator #2, he cut his steering wheel to the left to avoid the pedestrian and struck MV#1. No tows and only a minor injury to the operator of MV#1, though she did not want to be evaluated by the medics. Two independent witnesses to the crash gave me similar statements that operator #2 had driven on the wrong side of the road prior to the crash and was operating at a high rate of speed. Operator #2 cited for failure to obey signage. Only after I completed this report, was I notified by Cheryl Petitti, the mother of Marisa Petitti that Marisa was not operating MV#1 at the time of the crash and that she may have been struck by MV#2. I re-interviewed both witnesses and they confirmed that Marisa was in fact the pedestrian crossing the street, which I did not know, but they did not witness her being struck by MV#2 due to their view being obstructed. Cheryl Petitti also states that she was contacted by another witness, who had left prior to my arrival, that has information to provide. At this time I will notify the Traffic Bureau of a possible Auto Pedestrian to be further investigated.

The operator of MV1 stated he was traveling North on North street when he didn't see MV2 parked on the right hand side, facing north bound and struck the rear driver side of the vehicle. After striking MV2, MV1 crossed into the south bound lane, where it stopped. The airbags deployed and the operator was checked out by the medics and he signed a refusal. Both vehicles were un operable and the vehicles were towed from the scene. MV2 was unoccupied during the accident.
