On January 1, 2019 at approximately 02:18 hours I, Officer Guarino, responded to 78 Vine St. for a hit and run. I spoke to the operator of MV1, who told me that he was driving Eastbound on Vine St, he turned on his left directional and began to pull into his driveway at 78 Vine St. The operator of MV 2 attempted to pass MV 1 on the left hand side crashing into MV1. MV2 put the vehicle in reverse and left the scene. The operator of MV1 said that the front bumper of MV2 had fallen off in the collision with MA License plate 4KZ917 still attached. The owner of that vehicle lives in Boston, so Boston Police were notified. I attempted to make contact with the owner of MV 2 with negative results. Update January 1, 2019 16:15 hours: I was able to contact the owner of MV2, who admitted that she was the operator of the vehicle and that she was in the vehicle alone. The operator of MV2 said that the operator of MV1 was driving very slow and stopped in the right lane so she attempted to pass the vehicle. MV1 began turning and MV2 struck MV1. The operator of MV 1 said that she was nervous and left the scene because her phone died and attempted to contact the Newton Police in the morning. I am mailing citation T1441275 to Cecelia Spangenberg for MGL90/24C, leaving the scene of property damage. Photographs of the bumper have been submitted to IT to be attached to this report. The license plate was logged into evidence and placed into property and evidence locket #8.

On 1/5/19 at 1210hrs I was dispatched to the Pelham St Municipal Lot (public way inviting access maintained by the City of Newton) behind CVS for a report of an MVA involving several vehicles. On arrival, I observed Mv#1 MA Reg 5TWS10 with heavy front end damage wedged in between two other vehicles parked in marked spaces. #1 operator, in the driver's seat, was being evaluated by Medics. #1 was identified as Mary Jane Hymanwegscheider (10/14/38). I first made contact with a witness identified as Sharon Corliss. Ms Corliss stated she was sitting in her parked vehicle and observed Mv#1 backing out of the end spot next to CVS. She explained once #1 had came to a stop from backing, #1 then pulled forward, drove over a curb island and continued driving straight into the two parked vehicles. Ms Corliss stated it appeared #1 was travelling about 5mph preceeding the crash. After the medics intial evaluation of Ms Hymanwegscheider, I was able to ask her what happened? She stated she backed out of her space, hit the curb going forward and then put her foot on the gas instead of the brake before the collision. She was deliberate when speaking and appeared confused. She reported to medics she had no medical history and was not using any medications including allergy medication as well. She reported she did not hit her head after the collision. She was then transported to NWH for further evaluation after medics discovered her blood/sugar readings were very low. I advised Ms Hymanwegscheider her vehicle had heavy damage and was being towed by Tody's and to expect paperwork at the conclusion of the crash investigation. It appeared #1 struck Mv#2 first on its driver's side rear before striking Mv#3 parked next to it on the passenger side rear. #1 wedged itself between #2 and #3, #3 was then pushed to the left into the passenger side of Mv#4. #2 and #3 sustained heavy rear end damage. #4 sustained light damage. #1 towed by Tody's with heavy front end damage. #2 arranged for her own tow through her insurance company. A note was left on #3, and #4 was able to be driven from the scene. Ms Hymanwegscheider is showing no MV infraction/no accident history. Based on the totality of all information received as a result of this investigation, I will be issuing a citation (#T0340410) to Ms Hymanwegscheider for Ch90/Sec24, Negligent Operation.

On Saturday January 5th 2019 at 18:01, I was dispatched to Crafts St at Waltham St for a report of a 2 car accident. Operator of MV1 stated he was driving southbound on Waltham Street and approached the intersection as the traffic control changed to red. He slowed down and came to a complete stop. Approximately 4-6 seconds later, he was rear ended by MV2. MV1 sustained minor damage to the rear passenger side. MV1 had three other occupants in the vehicle at the time of the accident. The two passengers in the back seat complained of back pain and were evaluated by Cataldo Paramedic Basic 3 and both signed patient refusals. The operator of MV2 stated as he approached the intersection he "was sleeping." When I asked him to elaborate further he was unable to provide me any answers. He was evaluated by PB3 and transported to NWH. He was issued citation # T1441363 for Operating Under the Influence of Alcohol (2nd Offense) and Negligent Operation of a Motor Vehicle. See Report #19000596 for further details. MV2 was towed by Todys Towing due to the operator being transported to the hospital and the passenger stating he was too intoxicated to drive. A towed motor vehicle form was filled out.

On Saturday, January 12th 2019, at approximately 4:12pm, I, Officer Brooks, responded to a single vehicle accident in the area of Cherry street and Derby street, both of which are public ways in the city of Newton. Upon arrival in the area I observed the fire department off with MV1(MA REG 9PAK20), in front of 171 Cherry street. The vehicle had a fire hydrant lodged under the front axle, and front end damage. The fire department was attempting to wake the operator who was unresponsive. The fire department ended up administering 2 doses of Narcan to the operator before he became responsive. The operator was transported to Newton Wellesley Hospital by Cataldo for further evaluation. Based on the accident scene, it appears the operator was traveling southbound on Cherry street, crossed over the center line into oncoming traffic, striking the fire hydrant in front of 154 Cherry street, and then crossed back over into the correct lane of traffic before coming to rest in front of 171 Cherry street. The vehicle was towed by Tody's towing, and an inventory form was filled out and filed. The operator was mailed MA Uniform Citations T0648078 and T0648079 charging him with MGL 89/4A Marked Lanes violation, 90/24 OUI Drugs, 90/24(2)(A) Operating to Endanger/Negligent operation, 90/11 No License in Possession, and 90/11 No Registration in Possession. Officer Donahue took pictures of the fire hydrant and crash scene, which have been submitted to the IT Bureau. The fire department was made aware of the broken hydrant.

Narrative:
MA commercial reg M94754 (vehicle #1), a City of Newton pickup/sander truck, was located crashed over the curb in front of 1970 Beacon St. The vehicle was unoccupied, had the keys in the ignition, and was reported stolen just a few minutes prior to discovery. The reporting party of the stolen truck and uninvolved occupant is identified as City DPW worker George Sementelli. The witness/passerby who discovered the collision is also a DPW worker, identified as Donny Brandt. Pursuant to an investigation outlined in incident #19002213, the operator at the time of the crash was determined to be Michael Carter. It is believed that Carter stole the vehicle from 295 Auburn St, fled the area, and ultimately crashed in front of 1970 Beacon St. Corroborating facts outlined in the incident report put Carter as the operator. The vehicle itself was located partially on the sidewalk and partially on the road, therefore Carter is being cited with MGL 89/4A - Marked Lanes Violation (T1268423). Due to the fact it is believed that Carter fled the scene after the crash, MGL 90/24/C - Leaving the Scene of Property Damage was added as well as MGL 90/23/D Operating After a Suspended LIcense, and MGL 90/24/O - Reckless Operation of a Motor Vehicle. The crash occurred during busy weekday commuting about 0.7 miles from Angier Elementary School. The last 3 charges mentioned have been listed on a second citation (T1268422). Officer Brooks also issued T0648082 for OUI Drugs MGL 90/24/G. See the incident report for details about this charge. In total, there are (3) citations issued to Carter. The vehicle sustained heavy front end damage. It should be noted that the utility pole a couple of houses down from the crash site (Beacon/Varick Rd, pole #212) sustained fresh, minor superficial damage, consistent with the vehicle crashing into it before verring onto the sidewalk. Pictures have been taken and submitted to the IT bureau. Tody's responded and towed the vehicle to the police garage for processing due to the pending other criminal charges involved in this accident/incident. Michael Carter was subsequently placed under arrest and his citations have been placed into his property. Carter suffered a hand injury due to the crash an was transported to NWH Hospital for an evaluation after he was booked.

On 1/19/19 I responded to 25 Normandy Rd for a report of a past hit and run. I spoke with the Operator of MV#1 who stated the accident had taken place in front of Tom's Pizza, 423 Lexington St, at approximately 1830 hrs. Operator of MV#1 stated she parked her vehicle in a parking spot directly in front of the restaurant and went inside. When she returned approximately 5 minutes later she realized her vehicle had been rear ended. Two witnesses provided her with information about the accident and the plate number. (See witness narratives.) MV#1 suffered moderate damage to the rear and was driven from the scene. Waltham PD responded to the address listed for MV#2 and were not able to locate the vehicle. At approximately 2000 hrs, I was contacted at the police station by the owner of MV#2's wife. After further investigation (see incident report #19002425) it was determined that the operator of MV#2 at the time of the accident was the vehicle owner's brother. I spoke with Operator of MV#2 on the phone and he stated that the road was icy and slippery due to snow and he crashed into the back of MV#1. He stated he was nervous and thought he had only hit the sidewalk curb. Operator of MV#2 was issued citation T1440626 by mail for 90/24C Leaving the Scene of Property Damage, 90/10A Unlicensed Operation and 90/24 Use of MV Without Authority. He stated the vehicle suffered minor damage to the front.

On Sunday, February 3rd 2019, at approximately 11:45pm, I, Officer Brooks, responded to a single car MVA at the intersection of River street and Auburndale avenue. Upon arrival I observed MV1(MA REG 7TT558) in the travel lane with both tires broken free from the axle. I spoke with the operator who could not recall how the accident happened, or what was struck. After an investigation the operator was placed under arrest for 90/24 OUI Alcohol. Officer Boyle was able to assist in the accident investigation. It appears the operator had been traveling westbound on River street and struck a utility pole on the drivers side front tire. The vehicle continued to the intersection of River street and Auburndale avenue and struck a curb here on the passenger side tire. Both tires were broken free from the axles. The vehicle was towed by Tody's and an inventory form was filed. Eversource was notified about the pole.

Narrative:
At approximately 0110HRs on Sunday February 17, 2019, I was dispatched to the area of Dedham St. and Spiers Rd. (both public ways in the City of Newton), for a report of a motor vehicle vs pole accident with injury, airbag deployment and entrapment. Upon arrival, I exited my cruiser with my medical bag and AED and went to assist the motorist involved in the crash. MV1 (MA Passenger: 255ZA9) was completely off the roadway and on the shoulder of the road (facing Eastbound) with its nose contacting the light pole ( Verizon Pole #:103/80), which was now broken and listing to the side. Attempts to open the doors were unsuccessful and I attempted to communicate with the operator through a partially broken driver's window frame which was bent. The operator was slumped forward with his hand cradling his head and he was unable to speak, but was breathing. Newton Fire arrived and attempted to gain access to the vehicle via several manual tools with no success and instead deployed the Jaws of Life which was successful. Newton Fire also made entry into the rear passenger compartment because we viewed a child's car seat and checked with negative results for any child in the vehicle. Once the operator was cut out of his seatbelt and safely out of MV1 and on board Newton Medic 1, I entered the ambulance where he was identified as Mark Topham. It should be noted that Topham was having difficulty answering basic questions such as his name, date of birth, today's date, Etc. At one point when he was asked what today's date was, he gave his birth date of February 21, 1983 and it took him several attempts to say his last name, I had to run him over the air with a partial name to ascertain who he is. Also of note, in my presence and that of Sgt. Devine, Topham informed the medics that he had been consuming alcoholic beverages prior to the crash. Upon viewing the scene, I noted that the roadway was lit and I did not observe any skid marks on the dry, ice-free payment, indicating that there was unlikely any braking or swerving to avoid the collision. I spoke to the reporting party, Lindsay Giardina, she stated that she was traveling on Dedham St. when she heard what she believed to be a motor vehicle crash. When she came upon said crash, she attempted to assist the operator, as she was an off duty firefighter. She was unable to gain access to the inside of the vehicle, but communicated to the operator in the same manor as I did. She stated that she was able to smell the odor of an alcoholic beverage emanating from the operator. Topham was transported to Beth Israel of Boston, where he was assigned patient record number 3109921. I attempted to speak with him at the hospital, but was unable to do so because of his current state; I noted several injuries on Topham, such as a bloody lip, a wound to his lower left side and a wound to his left leg. Due to Topham's statements onboard of Medic 1 and Lindsay Giardina's observations, I will be charging Topham with one count of violating MGL Ch.90 S. 24 K OUI Liquor 2nd Offence, one count of violating MGL Ch.90 S. 24 E Negligent Operation of a Motor Vehicle and one count of violating MGL Ch. 89 S. 4A Marked Lanes Violation via MA Criminal Citations which I placed in the mail. Additionally, I will be seeking a subpoena for Topham's blood work from the hospital. Note: The crash occurred on the Newton/Boston line, Newton dispatch's GPS indicated that the crash was in Newton. I took several pictures of the scene and have forwarded them to IT to be attached to this report. I completed and filled an NPD Motor Vehicle Tow Inventory Form and Officer Boudrot stood by for the tow.

On 04/07/2019, at 2042 hrs, I responded to the area of Washington St at Centre St for a H&R MVA. Upon arrival, MV1 was pulled over in front of 317 Washington St. Its front bumper and undercarriage guard had been ripped off. The operator stated that as he was proceeding through a green light at the end of Mass Pike exit 17 onto Washington St (WB), a dark colored Jeep ran the red light on Centre St (NB) and struck his front end. The Jeep then proceeded down Centre St towards Watertown. A witnesses to the accident, Adam Nussenbaum, was traveling behind MV1 and corroborated the story. No further description of the vehicle or driver which fled was obtained except that it would have passenger side damage. Watertown PD was notified and a canvass of the area yielded negative results. I was unable to locate any cameras in the area of the accident. No injuries were reported and the vehicle was able to be driven from the scene.

On 04/08/2019, at 1620 hrs, I responded to the area of 2040 Commonwealth Ave for a three car MVA with injuries. Upon arrival, I observed all three vehicles facing westbound on Commonwealth Ave. MV1 had heavy front end damage and airbag deployment. MV2 also had heavy front end damage and airbag deployment. MV3 had minor rear end damage. The operator of MV1 was brought to Newton Wellesley Hospital by Cataldo medics. The operator of MV2 stated she was on Pamela Ct attempting to take a left onto Commonwealth Ave. There was heavy traffic headed westbound and a vehicle stopped to let MV2 go. As MV2 was taking a left onto Commonwealth Ave, MV1 was traveling westbound in the eastbound lane around traffic and struck MV2. MV2 then in turn struck MV3s rear end. The operator of MV3 stated that he wasn't sure what occurred because the accident was behind him. The operator of MV1 stated she was traveling westbound on Commonwealth Ave and was planning on taking a left into the Star Market parking lot which was approximately 250 feet down the street. She stated she thought there were two lanes in the westbound direction at that point in the roadway. MV2 then pulled out in front of her and they collided. Two witnesses who were traveling westbound in traffic a few vehicles behind MV3 both stated that MV1 drove erratically into the oncoming lane and around traffic heading westbound. One witness stated "the car was almost completely in the opposite lane" and the other witness agreed. A moment later they stated they heard the crash but did not see it. They also stated that she almost crashed into a car turning eastbound out of the Star Market exit prior to the accident. MV1 and MV2 were towed from the scene by Tody's. The operator of MV2 was brought to Newton Wellesley Hospital by her mother. The operator of MV1 was cited for C90 S23 operating after revoked license, C90 S23 operating after revoked registration, C90 S24 operating to endanger, and C89 S4a marked lanes violation. All aforementioned streets are public ways maintained by the City of Newton.

On 04/23/19 at approximately 14:35 Officer Gabriel and I responded to #150 Needham St- The Upper Falls Discount Liquor Store for a crash involving two vehicles. Veh #2 backed into parked veh #1. There was slight damage to veh #1 but moderate damage to veh #2. It was later determined operator of veh #2 was operating under the influence of alcohol when she backed into parked veh #1. Veh #2 was towed from the scene by Tody's towing after Officer Gabriel completed a towed vehicle form. Veh #2 registration was queried and came back revoked for insurance. The operator of veh #2 was arrested (NPD Report #19015765) and given in hand Mass. Citation's # T1444225 and T1444226 for Ch 90/Sec 24/J-O.U.I. Liquor, Ch 90 / Sec 24/E- Negligent Operation of a M.V., Ch 90/ Sec 34/J- Operating with out Insurance and Ch 90/ Sec 23 Operating after Registration Revocation.

On 5/20/19 at about 1510hrs I was dispatched to 136 Evelyn Rd for an MVA. En route to the location with Ofc Gourdeau, it was determined by first arriving Ofc Gourdeau that a vehicle had crashed through a fence at the end of Troy Lane and came to rest in the backyard of 126 Evelyn Rd. On arrival, I located the vehicle operator, identified as Mr Stuart Phillips who was operating MA Reg 8VVP40, an '03 Jaguar S Type model. Mr Phillips was walking around the backyard after the collision and appeared coherent as to what had transpired. He sustained a laceration to the top of his head and was bleeding heavily. We sat him down in a lawn chair prior to medics arrival to evaluate. When asked what happened, he stated he "hit the gas instead of the brakes". I asked him where did the accident begin? He stated he was turning around at the end of Troy Lane (a dead end cul de sac) and crashed through the fence there. Mr Phillips was alert and speaking clearly. I asked him if he was on any medication? He stated he was taking Plavix only, a blood thinner. I asked about his medical history and he stated he was released from Spalding Rehab today for treatment from a stroke and was receiving occupational therapy. Medics arrived to evaluate and he was transported to NWH for treatment. An investigation of the crash path of the vehicle revealed the following based on tire tracks and damage left behind: The vehicle crashed through the fence at the end of the cul de sac belonging to 41A Helene Rd; Vehicle drove over that back lawn, clipped the back deck attached to the rear of the house; damaging the base of the deck and lattice below; Vehicle travelled over the patio, then crashed through shrubbery down an embankment leading to the backyard of 132 Evelyn Rd; Vehicle drove through backyard of 132 Evelyn, then continued along the side of the house where it drove over damaging three retaining walls and clipped a large tree; After driving over the third retaining wall, vehicle continued through the backyard of 126 Evelyn, causing significant lawn damage; Vehicle continued until it hit the back of the house at 126 Evelyn; there, it drove over the rear patio before it struck a ground level basement window and window well; It came to a rest there where the vehicle caught on fire under the hood. Witnesses helped extinguish the fire with a garden house (see incident report #19020129) until NFD arrived to knock it down. Significant front lawn damage to 126 Evelyn occurred as well as NFD was arriving on scene to what was first reported as a structure fire. Several photos of the area were taken by myself. The memory chip was TOT the I.T. bureau to be attached to this report. Tody's towing towed the vehicle out of the backyard by accessing the rear driveway of 132 Evelyn Rd. Mr Phillips cited for Negligent Operation and an Immediate Threat for License Suspension/Revocation faxed to the RMV. Troy Lane is a public way in the City of Newton.

On 06/08/2019 at 0150 hours I received a call from Sgt Lee in regard to a head on motor vehicle crash with entrapment that had occurred on Center St at the four way intersection with Cabot St and Sergeant St. Upon my arrival at the crash scene I was advised by Sgt Lee that all occupants of the involved vehicles had been transported to hospitals for treatment. I made the following observations of the crash scene , Center St is a two lane public way in the city of Newton that runs north to south. I observed a city of Newton contracted Cataldo ambulance stopped in the south bound lane of Center St at Cabot St. The ambulance was MA AMN registration AMB 370 a 2017 Ford Econo Line that had heavy damage to the front end and driver's side fender. I observed that the front tire of the ambulance was snapped off and was pushed back towards the driver's side door. I further observed that both of the front and side air bags of the ambulance were deployed. The front of the ambulance's final point of rest was 8'8" from a fixed overhead light pole on the northeast corner of Center St at Cabot St. The ambulance's front passenger side tire was 4'3" front the curb on the Cabot St side. The ambulance's rear passenger side tire was 2'3" from the Cabot St side curb. The ambulance's rear driver side tire was 5'1" from the double yellow line on Center St. The ambulance's front driver side tire was 6'3"from the Center St double yellow line. The width of Center St from the double yellow line to the Center St curb is 15'' and 16'" to the Sergeant St side. I observed a 2011 Mazda 3 with MA PAN registration 6KJ783 facing south bound in the north bound lane. The Mazda had heavy damage to it's front end, and I observed that it's front tires had snapped off. The Mazda's roof had been removed by Newton Fire to extract the occupant. The front of the Mazda's final point of rest was 40'3" from a fixed overhead light pole on the northeast corner of Center St and Sergeant St. The Mazda's front passenger tire was 4'6" from the double yellow line on Center St. The Mazda's rear passenger side tire was 7'6" from the Center St double yellow line. The Mazda's rear passenger side tire was 1'2" from the Center St double yellow line. The Mazda's front driver's side tire was on the curb on the Sergeant St side of Center St. There were no skid marks from the Mazda which indicates that no braking occurred. At the time of the crash, the Cataldo Ambulance was being operated by Paramedic Corey Bohan and Paramedic Tom White was in the front passenger seat. Bohan stated that he was operating his vehicle southbound on Center St towards Cabot St after clearing a medical call. There were no patients in the ambulance at the time of the crash, and Bohan estimated he was travelling 25 MPH . Bohan stated within less than two seconds he observed a vehicle traveling towards him northbound on Center St in the southbound lane and estimated it's speed to be approximately 80 to 100 MPH.Bohan stated he was able to slow to a near stop and had moved his vehicle towards the right shoulder on Center St before the Mazda struck his vehicle head on. Bohan stated that after forcing his way out of the driver's seat of his vehicle, he rendered medical aid to the operator and only occupant of the Mazda, Jessica Pondish. Bohan stated that Pondish speech was slurred and he detected an odor of an alcoholic beverage emanating from her breath and person. Bohan was later transported to Newton Wellesley Hospital for evaluation. The passenger in the ambulance, Tom White stated to officers that he was working on the ambulance's laptop computer and did not see anything leading up to the impact from the crash. White stated that he was able to force himself free of the passenger side of the ambulance, and rendered medical aid to Pondish. White stated that Pondish was " in and out of it " and did not appear coherent.White stated that he asked Pondish if she had consumed any alcoholic beverage and she nodded in the affirmative. White Stated that he was unsure if Pondish understood his questions. White stated that he did not detect any odor of an alcohol beverage emanating from Pondish's breath or person. White was transported to Newton Wellesley Hospital for evaluation. Pondish was freed from her vehicle after approximately 22 minutes by Newton Fire Department units by utilizing special extraction tools. Pondish was transported by Newton Medic Unit Two to the Brigham and Women's Hospital in Boston for treatment. From 06/08/2019 to 06/10/2019 Pondish had two surgeries and was unable to speak due to her condition. On 06/10/2019 myself and Officer Gaudet were able to speak with Pondish in her hospital ICU room at the Brigham and Women's Hospital. Pondish's parents Lorena and John Pondish were present while we spoke with her. Officer Gaudet asked Pondish about the events that led up to the crash. Pondish stated that on 06/07/2019 she was at a friend's residence in Norton, MA. Pondish stated that while at her friend's residence she consumed a bottle and a half of Riesling wine. After Pondish's admission myself and Officer Gaudet stopped our conversation with her. I read Pondish her rights directly from a Miranda card and explained them in full before allowing her to continue speaking with myself and Officer Gaudet about the events of that led to the crash. Pondish stated that she understood her rights. Officer Gaudet asked Pondish if she had consumed any alcoholic beverages prior to driving home to her residence in Newton. Pondish stated that she drank a bottle and a half of Riesling wine by herself between the hours of 1700 and 0000 hours on 06/08/2019. Officer Gaudet asked Pondish how large the bottles of wine was that she drank, and she stated that they were average size. Officer Gaudet asked Pondish how she perceived her state of being was after consuming a bottle and a half of wine. Pondish stated that she felt " buzzed " . Pondish stated that she left her friend's residence in Norton at approximately 0100 hours on 06/08/2019 to return to her residence in Newton. Pondish stated that she does recall the events leading up to the crash, except that she tried to make a phone call but the person did not pick up. Pondish stated that her seat belt was on and that she was not texting while operating her vehicle. Pondish stated that her injuries consisted of a broken left elbow and internal injures to her intestines. On 06/09/2019 I spoke to Cataldo Paramedic Robert Canning and Stephen Heirholcer who were the medic unit that transported Pondish to the Brigham and Women's Hospital. Canning stated that he rendered medical aid to Pondish for approximately 20 minutes while Fire units worked to free her. Canning stated that Pondish's legs were pinned under her vehicle's dashboard. Canning stated that during the approximately 20 minutes that he was in close proximity to Pondish he could detect a strong odor of an alcoholic beverage that was emanating from her breath and person. Canning stated that once Pondish was freed he continued to treat and monitor her during the transport to the Brigham and Women's Hospital. Canning stated that when he asked Pondish if she had consumed any alcoholic beverage she stated that she had. Paramedic Heirholcer stated that he did not detect any odor of an alcoholic beverage emanating from Pondish's breath or person. Based upon the physical evidence at the crash scene, and statements made to officers by all of the above mentioned persons, Pondish is being charged with violation of MGLs 90/24j OUI liquor, 90/24e negligent operation of a motor vehicle, 89/4a failure to stay within marks lanes, and 90/17 speeding. On 06/12/2019 at the conclusion of the crash investigation I issued MA Criminal Application T 1269996 in hand to Pondish at her hospital room and explained to her what action she needed to take with court. On 06/11/2019 a letter of preservation was sent to Brigham and Women's Hospital requesting that a sample of Pondish's blood, urine and medical records be saved. On 06/12/2019 Brigham and Women's Hospital confirmed that a sample of Pondish's blood had been preserved. As of 06/12/2019 a search warrant for Pondish's blood sample was pending. Photos of the involved vehicles and of the crash scene were taken and downloaded by the NPD's IT Bureau. Both involved vehicles were towed from the crash scene and towed motor vehicle forms were filled out. An Iphone and backpack belonging to Pondish were taken from her vehicle for safekeeping and returned to her on 06/11/2019. A copy of this report was faxed to the MA RMV Driver Control Unit to request an immediate threat suspension.

On 07/24/19 at 08:06 hours, I responded to a two car motor vehicle accident at Dedham and Parker Streets with no reported injuries. Operator of MV1 was stopped at the stop sign on Parker Street (SB) at Dedham Street waiting to take a left turn on to Dedham Street. That is when he was struck in his rear end by MV2. MV1 sustained moderate damage to the rear bumper and hatch area but it was still driveable. Operator of MV2 states she was behind MV1 on Parker Street but did not see that vehicle in front of her. She states she was also attempting to take a left turn on to Dedham Street. She then started to accelerate and struck MV1 in the rear end. I observed moderate damage to the entire front bumper and grille but mainly to the driver's side. The owner decided to have the vehicle privately towed after on Howe Road. Operator was issued MA Uniform Citation # T1445344 for Newton City Ordinance 19-75 Failure to use care in starting and stopping.

At approximately 1825HRs on August 06, 2019, I was dispatched to 785 Centre St. (Newton Country Day School) in reference to a motor vehicle vs bicycle crash. Upon arrival, I located both parties on the sidewalk adjacent to the intersection of Centre St. and Colby Rd. The operator of the bicycle was complaining of feeling dizzy, although she stated she did not hit her head, and suffered several abrasions on her left leg and hands; the operator of MV1 stated she was uninjured. The operator of MV1 stated that as she was traveling northbound on Centre St., she attempted to give the operator of the bicycle room, but the bicyclist made an abrupt left hand turn and struck MV1. The operator of the bicycle stated she was traveling northbound on Centre St. in the Bicycle Lane. She then signaled a left hand turn and entered the Motor Vehicle Travel Lane to make a second left hand turn to travel eastbound on Colby Rd. Once she exited the northbound Motor Vehicle Travel Lane on Centre St. and began to cross over the southbound lane, MV 1 crossed over the solid yellow lines onto the southbound side of Centre St. and struck her bicycle. MV1 sustained damage to its right front bumper and I noted red paint transfer of the left side of the front bumper, which the operator of MV1 states was old damage. MV1 also sustained a flat left front tire after contacting the curb. The bicycle sustained damage to both tires and where the rear tire attaches to the frame appeared to be warped. I took several photos of the damage and have submitted them to be attached to this report. The operator of the bicycle was transported to the hospital by Newton Medics and the operator of MV1 signed a patient refusal. Todys Services arrived and took custody of MV1, for which I completed and filled an NPD Towed Motor Vehicle Form. All streets mentioned are public ways in the City of Newton. Due to the potential danger of serious bodily injury caused by the operator of MV1's reckless driving behavior (crossing the solid yellow road divider line and driving on the wrong side of the road against traffic), the actual damage caused (striking and causing the bicycle operator to be transported to the hospital and damage to the bicycle) and that the bicycle operator was clearly visible/in clear contrast with the road (riding a red bicycle and wearing a red shirt) I issued the operator of MV1 MA Criminal citation T1445010, in hand, for violating one count of MGL Ch.90 S. 24 (Operating To Endanger) I cleared without incident. Note: I logged the bicycle into property/evidence for safekeeping, as it could not be taken on the ambulance.

On Thursday, August 8th 2019, at approximately 8:25pm, I, Officer Brooks, was parked in front of 719 Washington street facing westbound. I looked into my side view mirror to pull into traffic and observed MV1 (MA REG 118GL4) parked behind me with the driver's side door open. The operator was leaning into the car to retrieve an item, and then stood up and turned around to close the door. I observed MV2(IN REG VWI565) traveling westbound, at what I estimated to be 45 miles per hour. MV2 struck the open door of MV1, narrowly missing operator #1. MV1 sustained major damage to the driver's side door and was towed by Tody's. MV2 had heavy damage to the passenger side front end. The operator of MV2 was placed under arrest for Operating under the influence of alcohol after investigation. MV2 was towed by Tody's. There were no injuries as a result of the accident.

MV1 operator, David Gorman (2017 Mazda Cx3, MA Reg:6TL761), stated that he was traveling from Commonwealth Ave turning right on Lexington St heading NORTH when he realized the vehicle behind him was following him very closely. MV1 continued to drive on Lexington St when MV2 (2015 Toyota Highlander, MA Reg: 6VS151), started to beep at him continuously and went over the double yellow line in an attempt to pass MV1. MV2 was unable to pass MV1 because of oncoming traffic so MV2 steered to get back into his lane. MV2 collided with MV1 in the area of 281 Lexington St. A witness, John Maclellan (off-duty police officer Watertown) who was in the area of Lexington St and Orris St. stated that he heard a vehicle beeping repeatedly and then heard a collision. Maclellan came outside to find MV2 over the double yellow lines facing oncoming traffic. MV2 had damage to its passenger front and MV1 had damage to its drivers side rear. Maclellan told MV2 operator, Renato Ribeirodasilva, to move his vehicle to a safe location on the right side of the road and get out of the vehicle to exchange paperwork with the other driver involved (MV1, Gorman). Maclellan phoned Newton PD to notify of the accident and as he was on the line, he witnessed both parties getting into a heated arguement on the sidewalk outside of their vehicles. MV2 operator, Renato Ribeirodasilva was taken into custody for the following charges: 90/24J - OUI Liquor, 90/24E - Neglegant Operator of a MV, and 89/4A - Marked Lanes Violation. Claudineia called a sober friend to drive her and her three children home to Waltham in Claudineia's vehicle. Claudineia was also under the influence of alcohol.

MV1 jumped a curb and collided with a parked vehicle MV2 (a 2004 GMC Yukon, MA Reg: 8EC792) in front of 185 Waltham St. MV1 operator, Angel Gonzales, was unconscious on scene in the driveway of 185 Waltham St. Waltham St is a public way that is owned and maintained by the City of Newton. A passerby, Joseph Anderson saw the aftermath of the crash and called 911. MV2 had damage to its drivers side door and rear side. MV1 was heavily damaged, on its side, and keys were in the ignition. MV1 was towed by towed by Tody's. Gonzales was transported to Beth Isreal in critical condition. When Gonazales was in the back of the ambulance, myself and medics smelled an odor of an alcoholic beverage emanating from Gonzales. All first responders thoroughly checked the surrounding area for a helmet but one could not be located. Due to Gonzales' critical condition and urgency to get to the hospital, and ID was not thoroughly looked for on his person. Ofc. Guarino, N491, retrieved an ID from Gonzales upon arrival at Beth Isreal Hospital. Ofc. Stake filled out a towed inventory form. Pictures taken and sent to IT. Sgt. Lee was notified and will be following up with this incident. MV1 operator, Angel Gonzales is being charged with the following (MA Uniform Citations T1442823 & T1442824): 90/24J - OUI Liquor (public way) 90/24E - Negligent Operation of a MV (operating recklessly at a high rate of speed) 89/4A - Marked Lanes 90/23 - Attaching Registration Plates (attaching registration plates to motorcycle to conceal the fact that it was unregistered. Registration plate on the motorcycle came back to a 2002 Honda VT750 motorcycle, MA Reg: 1T3988; VIN on the motorcycle came back to a 2005 Suzuki GSXR60, MA Reg: 1L4389) 90/9B - Unregistered MV (motorcycle 2005 Suzuki MA Reg: 1T3988 was confirmed unregistered by RMV) 90/23D - Operating MV with a Suspended License (RMV confirmed that Gonzales' license was suspended S94722414) (see Incident #19039806 for more details). For QED purposes, the charge 266/139 - Stolen Registration was added to this report because 90/23 - Attaching Registration Plates was not an option. Due to Gonzales' hospitalization, the citations will be mailed to his last known address. Officers were able to rise a Harold Curtis, the father of Anthony Curtis (owner of MV2, the GMC Yukon) at 185 Waltham St. He was notified of the incident and advised to call their insurance company. Curtis mentioned that he has cameras facing the street where the incident took place and will see if he can retrieve the footage for us. Police prosecutor to subpoena blood.

On September 30, 2019 at approximately 16:56 I responded to a reported hit and run crash at 195 Sumner St. Dispatch gave out a description of the vehicle that fled, described as a red pickup truck with a possible partial plate of" MVNA". I, along with several other units checked the area for the vehicle with negative results. I responded to 195 Sumner St. and spoke with the operator of MV 1, who explained that he was traveling Northbound on Langley Rd onto Sumner St when a red pickup truck attempted to pass the RP from behind. Due to Sumner St being a narrow, one way street, the pickup truck cut back in front of MV1 and struck the front driver's side of MV1 with the rear passenger side of the pickup truck.The pickup truck proceeded down Sumner St. towards Centre St. without attempted to stop. The operator of MV1 did not know the make or model of the vehicle, but described it as a full size pickup truck that was red/maroon with a black tailgate with a large "fox racing" sticker on it. The description of the vehicle that fled was broadcasted to units canvassing the area, but was not found.

Vehicle #1 was traveling north on Chestnut St. In the area of Chestnut St. and Oliver Rd. Vehicle #1 struck the curb on its passenger side. Vehicle #1 then crossed the double yellow lines crossing into the south bound travel lanes. Vehicle #1 then crashed into and over the curb and snow bank traveling completely off the road and into a utility pole on the sidewalk. Vehicle #1 was sustained heavy front end damage and was elevated off the roadway on top of a snowbank with a utility pole indented into its front end. The crash into the utility pole caused the pole to split in half. The operator of vehicle #1 was uninjured and refused medical attention. The operator of vehicle #1 was cited with MGL 90-24/K operating under the influence, MGL 90-24/E negligent operation of a motor vehicle, 90-24/C leaving the scene of property damage and MGL 89-4A marked lanes violation (citation #T2079135 & #T2079136). Operator of MV1 states she was traveling eastbound on Washington Street when she entered the intersection where Rt. 95 on/off ramps and Quinobequin Rd meet. Operator of MV1 states she had the green signal when MV2, described as a dark colored pick up truck, ran the red light traveling northbound from the off ramp of Rt. 95 southbound/Quinobequin Rd. and crashed into MV1 head on. Operator of MV1 states MV2 fled the scene traveling northbound in the Do Not Enter lane traveling towards Grove St. Operator of MV1 states MV2 sustained front end damage, while MV1 sustained damage to the right front fender and was towed from the scene. Operator of MV1 sustained a wrist injury and was transported to Newton Wellesley Hospital. While investigating the scene, the license plate (MA Reg: 1YF845) from MV2 was left behind from the crash. The registration plate came back on a gray 2017 Honda Ridgleline pick up truck, which matches the description of the vehicle involved in the crash. The vehicle is registered to Andrew Wiggin out of 32 Marlborough Rd Salem, MA. Two witnesses, Lisa Duncan and Jennifer McQuillan, witnessed the crash. Ms. McQuillan states she was behind MV1 when she observed MV2 run the red light, crash into MV1 head on and fled the scene northbound in the Do Not Enter lane. Ms. Duncan states she was a few car lengths behind MV1 traveling on the right hand side of MV1. Ms. Duncan also states she observed MV2 run the red light, crash into MV1 head on and fled the scene northbound in the Do Not Enter lane. Both Ms. McQuillan and Ms. Duncan believe a male was operating the vehicle.

The operator of MV1, Nicole Laurin, sates that she was driving southbound on Sheridan Street when she lost control of her vehicle causing her to crash into MV2 which was parked in front of 107 Sheridan Street. Both vehicles sustained major damage and airbag deployment and were towed from the scene by Tody's. Due to the high rate of speed of the crash and air bag deployment, the operator of MV1 was transported to Newton Wellesley Hospital to be evaluated. It should be noted that prior to the reported crash there was a 911 call regarding a white sedan traveling at a high rate of speed going through numerous traffic signals and signs in a densely populated neighborhood with narrow streets. After the crash several more 911 calls were received stating that a vehicle traveling at an excessive rate of speed just crashed into a parked vehicle. Based on these calls and the nature of the crash, Ms. Nicole Laurin was issued in hand Massachusetts Unifrom Citation T2013231 for 90-24, Operating to Endanger, and 90-17, Speeding over the reasonable and proper rate. Sheridan Street is a public way in the city of Newton.

At approximately 1645HRs on Tuesday, 01/07/2020, I was dispatched to 325 Boylston St. (Sunoco Gas) for a report of a hit and run motor vehicle crash. Upon arrival, I spoke with the operator of MV1 (MA Veteran:J706), who stated that as he sat stationary in his vehicle just before entering pump: 3, MV2 backed up from pump: 1 and struck MV1 in the front bumper, despite him activating his horn and keeping it active as she continued to back up. The operator of MV1 then approached the driver's side door of MV2 to exchange information, however the operator of MV2 drove off; the operator of MV1 provided MA Pass: 8SV842 being attached to MV2 and I noted damage to the front bumper of MV1. I spoke to witness 1 via phone and he concurred with the operator of MV1's account of the incident. It should be noted that witness 1 could see the crash well from his vantage point by pump: 2. The operator of MV2 could not be reached by this officer for comment. Natick PD responded to the operator of MV2's residence and observed damage to MV2's rear bumper. They requested the operator of MV2 contact me via phone and she stated she would; Natick PD did not get a contact number for her. Natick PD informed us to expect her call and a few minutes later, dispatcher Kelly stated that she received a call from the operator of MV2, who immediately hung up once the call was answered. It should be noted that since she called on the business line and immediately hung up, we were not able to retrieve her phone number as the system does not store it. Due to the operator of MV2 failing to provide her information after the crash/fleeing the scene and her not being cooperative with my investigation, I have issued her by mail MA Criminal Citation:T2013367 for violating one count of MGL. Ch. 90 S. 24 (Leaving The Scene Of An Accident After Property Damage). Note: the clerk at Sunoco stated that the cameras at the pumps do not store footage. **Update** At approximately 1540HRs on Thursday, 01/07/2020, I spoke with the operator of MV2 via telephone. She contacted the Newton Police Traffic Bureau and provided her phone number, who in turn forwarded it to me. She stated to me that on the night in question, she was at the pumps and the pump she initially wished to use (pump: 1) had a sign on it to pay inside. She then elected not to use said pump and backed her car up and heard the operator of MV1 activate his horn. She stopped and was unaware that she struck MV1. When she saw the operator of MV 1 exit his vehicle, she assumed he was going to pump gas. She then stated she left the gas station because she was frustrated and did not intend to leave the scene of an accident. I advised the operator of MV2 that since I was unable to speak with her that night, I had already issued her MA Criminal Citation:T2013367 for violating one count of MGL. Ch. 90 S. 24 (Leaving The Scene Of An Accident After Property Damage) via mail and advised her how to proceed.

On Thursday March 26, 2020 at approximately 0218 hours I responded to 84 Derby st for a report of a car accident. Upon arrival a Blue Honda Sedan MA reg 1HLk36 was on the street outside of 84/86 Derby St with major damage to the front and rear of the vehicle. The vehicle was unoccupied and came back to 86 Derby St. I spoke to the resident, Dawna Noguira, who stated her 14 year old daughter Sabrina Dirgham was the operator of the vehicle. I Then spoke to Sabrina who stated she took her mothers car without her permission. Sabrina stated she reversed out of the driveway and drove over the sidewalk across the street and hit the wall at 79 Derby St. She then stated she attempted to turn but accelerated too fast which caused her to hit her neighbors side of the duplex and vehicle MA Reg 312FG7 at 84 Derby St. The impact of the collision with the house caused the vehicle to roll back into the middle of Derby street. Sabrina then fled from the vehicle, abandoning it in the street, and going into her house because she was scared. Sabrina declined medical attention and her mother signed a refusal. Vehicle 1 was towed from the scene by todys. Sabrina was given in hand MA Citation T2012818 for 90/10 Unlicensed Operation, 89/4A Marked Lanes Violation, 90/24C Leaving the Scene of Property Damage and MA Citation T2012819 for 90/24 Use Without Authority and 90/24E Negligent Operation. A message was left with the city building inspector to check the house tomorrow. Pictures of the damage of both vehicles, house and wall were submitted to IT.

MV1 operator was unsteady on his feet, bloodshot and glassy eyes, slurred speech, and had an odor of alcoholic beverages coming from his breath. MV1 operator stated he was driving westbound on Commonwealth Ave and then took a left onto Lexington St southbound. MV1 operator stated he was then struck on the front passenger bumper by MV2. MV1 operator at the time reported no injuries. MV1 sustained heavy front end damage to the front passenger wheel well and was towed by Todys. MV #1 was traveling EASTBOUND on Webster St (a public way as it is maintained by the City of Newton) when it collided with a curb then a stone wall (private property belonging to 239 Webster St). MV#1 operator, Jordan Feinberg was found in front of 174 Webster St sitting on the sidewalk. He had a laceration on his arm and a lump forming on his forehead. Feinberg said that he drank half a bottle of wine tonight and hit a curb down the road (Webster St). Feinberg was sweating profusely, unsteady on his feet and had a very strong odor of an alcoholic beverage on his person. He said he was the only person in the vehicle. Officers went to go observe the vehicle and it had major front end damage and had collided with a stone wall belonging to 239 Webster St. When asked if he hit a stone wall, Feinberg said "no, I just hit a curb". He had no memory of hitting a stone wall. Resident of 239 Webster St was also the reporting party, James Pelis, said that he heard tires screeching and then a crash. He came outside and saw a MV up on the sidewalk and a male individual approximately 250lbs, heavyset, grey sweatpants and a dark shirt walking Eastbound on Webster St away from the crashed MV (this description fit Feinberg). Pelis assumed it was the driver but never saw him get out of the car. Feinberg was transported to NWH by medics. The vehicle (belonging to Feinberg's grandfather, Frederick Shapiro) was towed by Tody's. A towed motor vehicle formed was filled out. Pictures of the scene, damage to the stone wall and the MV condition were taken and the SD card was placed into IT's box. A criminal application was issued and given in hand to Feinberg at NWH by Ofc. Stake. The criminal application included 90/24J - OUI Liquor, 89/4a - Marked Lanes Violation, and 90/24E - Negligent Operation of a Motor Vehicle.

On Thursday 04/16/2020 at approximately 2306 hrs I was dispatched to a hit and run accident in the area of 320 Washington St. Upon arrival I observed debris all over the roadway including a City one way street sign, a City utility light pole along with a Metropolitan MWRA vent. The RP mentioned to dispatch he observed headlights driving away from the scene and believed it was a black vehicle once he exited his residence and observed the vehicle parts in the roadway.The vehicle later identified as a 2020 Toyota Rav 4 black in color bearing MA.REG (97E520) was not on scene of the accident. Shortly after I responded on scene the vehicle involved was located nearby by N-494 at the intersection of Washington St. at Adams St. The vehicle was registered to a Evgenii Shatokhin with heavy right front end damage along with the left front, and both rear tires deflated. Mr. Shatokhin stated he was the operator of the vehicle and I asked what happened back at 320 Washington St. He stated he was driving Westbound on Washington St. when he swerved to avoid something and hit the curb. Once he hit the curb, he stated he then hit the sign, light pole and MWRA vent. Due to Mr. Shatokhin not remaining on scene of the accident and leaving he is being charged with Ch. 90 Sec. 24 (Leaving the scene of an accident, property damage). Failed to remain on scene after accident occured involving property damage. He is also being charged with Ch. 89 Sec. 4A (Marked Lanes) for failure to drive entirely within. Citation #T2014768 was given in hand to Mr. Shatokhin. MV1 sustained heavy right front end damage. No injuries. Tody's responded on scene and towed MV1. A towed motor vehicle form was filled out and placed on file. Pictures were taken on scene of the property damage and placed into the IT Bureau's box.

The OP. of MV#1 was unable to give me a statement due to a possible head injury. He kept repeating "where's my car" and "what happened?" Witness#1 (Mr. Regan) states he was travelling westbound on Auburn St. when MV#1 went to the oncoming traffic lane and passed him on the left at a high rate of speed. He estimates the speed at 70mph. He saw MV#1 hit the speed bump on Auburn St., possibly losing control, and then onto Evergreen Ave. with a large cloud of dust. Witness#2(Mr. Weishaus) was on his balcony and observed MV#1 travel at a high speed of rate on Auburn St. He estimates the speed at 100mph. He observed MV#1 go up Evergreen Ave., launch into the air and crashed. Witness#3(Mr. Litvak) states he was on his porch and heard a loud sound of a motor engine approaching. MV#1 then crashed into his retainer wall and flew through some trees before landing on his neighbor's yard. Based on witness statements and the accident scene, it appears that MV#1 went off the roadway and struck a retainer wall at #15 Evergreen Ave, launched into the air and struck parked MV#2 in the process, went through some trees that flipped the car around before landing on the property of #23 Evergreen Ave. The OP. of MV#1 was ejected from the vehicle at some point in time. Pictures were taken of the accident scene.

Based on observations, and statements made, the following occurred. M/V#1 had been traveling east on Needham St. and was being followed by M/V#2 in a road rage incident. (see NPD incident report #20032561) M/V#1 pulled into 165 Needham St. and parked in front of Stop N Shop. M/V#2 parked a few spots away. All occupants of both M/V's exited the car and yelled at each other. The passenger of M/V#2 (identified as Rene Marquez ) threw a bottle at the occupants of M/V#1 and missed, he then approached them and tried to punch one of them, and missed. All parties then entered their respective M/V's. M/V#1 backed out of its spot, and Marquez got in the drivers seat of M/V#2, backed the M/V up, then drove into M/V#1, before backing up again, and leaving the scene. I spoke to the owner of M/V#2 several hours later, and she stated that she was driving, was scared, and accidentally hit M/V#1. She stated that she was going to go to Needham P.D. to report the incident and accident, but "just didn't". All witnesses state that Marquez was the operator of M/V#2 at the time of the collision.

Operator #1 states that he was taking a left onto Lexington St from Wolcott St when his vehicle (vehicle #1), MA 1ZDP33, was hit by a white Chevrolet pickup with unknown plates. Operator #1 states that he had a had a green light but was able to swerve out of the way to minimize contact when the white truck ran the red light southbound on Lexington St. Damage is to the front right corner of vehicle #1. The white pickup did not stop. It turned right onto Commonwealth Ave towards Weston. I spoke to witness Sean Murphy (508-661-9329) who heard the crash and observed the immediate aftermath of the pickup taking off. He provided a further description of a 2 door white Chevrolet pickup, early 00's model, occupied 2x, with the name Empire Concrete on the passenger side. I attempted to locate an Empire Concrete, however limited search results were available. A possible business address of 54 Forest St Mllford was found and I had Milford PD respond to the address to look for the pickup. It was not there, and I was informed that the address itself is an apartment complex. It should be noted that there is no phone number or website for an Empire Concrete out of MA, although there are several other businesses that have the word Empire in the company name. The camera facing Lexington St at Gregorio Jewelers (corner of River and Lexington) did not yield any matching results for the appropriate time frame. Starbucks (corner of Commonwealth and Lexington) doesn't have outdoor surveillance equipment. Vehicle #1 did not need to be towed and no medical attention was requested by the operator. He was advised that if I am able to obtain further information or verify the company, I will contact him.

On Friday, September 18, 2020, at 88update88approximately 0657 hrs, while assigned to N495, I was dispatched to the area of 1221 Centre St for a report of a MV accident. When I arrived on scene I observed a Motor Cycle (MC) laying on its side in the middle of the street. I observed a party laying on the ground, while medical personnel were tending to him. I also observed a white Honda (MV#1) pulled off to the side of Pelham St, and the operator standing next to his vehicle. Operator of MV#1 stated that as he was attempting to make a left turn off of Centre St, onto Pelham St. Operator of MV#1 stated that while he was in the middle of his turn, he observed a MC driving at a high rate of speed coming towards him. Operator of MV#1 stated that he was not able to get out of the way of the MC before it crashed into him I was not able to speak with the operator of the MC, due to him being transported to Brigham and Womens Hospital. Ofc McSweeney stated that he will be following up with the operator of the MC and will advise me. It should be noted that the operator of the MC appeared to be wearing headphones while operating, picture was taken and submitted. An anonymous party stated to me that he observed the MV traveling at a high rate of speed, heading North on Centre St. The party stated that he observed the MC weaving in and out of traffic. Multiple pictures were taken of MV#1 and MC and were submitted to IT to be attached to this report. Operator of MC was transported to Brigham and Womens. Operator of MV#1 was cleared with a patient refusal. Both vehicles were towed by Todys. **UPDATE** I spoke with the father of Scott Mahon, Keith Mahon. Keith stated that his son is still in the hospital, and has received several surgeries. Keith stated that the injuries his son received are a Open Tibia/Fibula fracture of the right leg, and a concussion. Keith stated that Scott has no recollection of the incident, as far as to not remembering leaving the house that morning.

On 09/30/2020 at approximately 19:32hrs members of the Newton Police Patrol Bureau responded to a motor vehicle crash near the intersection of Washington St and Eddy St. After an initial investigation of the crash the Patrol Bureau secured the crash scene and called in the Newton Police Traffic Bureau to investigate. At approximately 19:45hrs Sgt. Devine, Ofc Wilson and I (Ofc Schlegel) from the NPD Traffic Bureau responded to the scene to investigate. Upon arrival to the scene I was met by Ofc Kane of the NPD Patrol Bureau. Ofc Kane stated that the crash was located in the driveway of 74 Eddy St and that it involved a Ford F350 pickup striking two parked cars. Ofc Kane stated that the operator identified as Kyle Butler had already been taken to Newton Wellesley Hospital by Cataldo EMS for injuries. . Upon approaching the crash scene I observed the following: The crash occurred at night time hours at approximately 19:32EST near the intersection of Washington St and Eddy St. Both Washington St and Eddy St are public ways in the City of Newton. The weather conditions at the time of the crash were a dark and clear with a temperature of approximately 60 degrees F. Washington St is a four lane undivided roadway (2 westbound travel lanes and 2 eastbound travel lanes). Washington St lighted roadway which has concrete light poles on both the westbound and eastbound sides. Washington St was under construction at the time and had a base coat of asphalt applied. At the time of the crash Washington St did not have any painted lane markings due to ongoing construction. Washington St is separated from the road sides on both the eastbound edge and the westbound edge with fixed granite curbing. The roadway also had several orange traffic barrels and traffic cones located edges of the eastbound and westbound lanes. Affixed to the light poles and the traffic barrels were temporary no parking signs for the ongoing road construction. At the time of the crash no road construction activities were taking place. No major debris or other obstructions were observed in the roadway. Washington St is a major roadway in the City of Newton and travels west to East across the entire City of Newton. This area of Washington St where the crash occurred is in between West Newton Square and Newtonville Square, this area has a posted 35 MPH speed limit. Both West Newton Square and Newtonville Square are densely populated with several businesses and residential areas. The section of Washington St where the crash occurred is also traveled frequently by pedestrian, bicycle and motor vehicle traffic. At the time the crash occurred 19:32hrs This area of Washington St is still a heavily travelled roadway. I observed Vehicle 1 described as a 2003 Ford F350 color white (MA Reg 5DT664) located in the driveway of 74 Eddy St. 74 Eddy St is a multi-family dwelling house located on the corner of Washington St and Eddy St. The driveway of 74 Eddy St is on the Washington St side. Vehicle was faced easterly across the driveway. I observed Vehicle 1to have extensive damage to the entire vehicle including but not limited to the enclosed cab area, cab roof and all windows, front end, rear end, along with both driver and passenger side. I also observed city of Newton owned concrete traffic light resting on top of the cab area of Vehicle 1. Upon further observation of Vehicle 1 it appears that both front tires and rims were flat, heavily damaged and had little to no tread left. Both front tires were worn down to the interior steel belt. I also observed that Vehicle 1 did not have a valid inspection sticker. Located to the immediate front of Vehicle 1 and resting on the front end was a white fence which had damage consistent with the impact of a motor vehicle. Immediately to the driver's side of Vehicle 1 was Vehicle 2 described as a 2007 gray Subaru Forester (MA Reg 45D610. Vehicle 2 was facing westerly across the driveway. Vehicle 2 had extensive damage to the driver's side front end, rear end, in addition to the entire driver and passenger sides. At this time Vehicle 2's drivers door was in contact with Vehicle 1's drivers door. Both drivers doors could not be opened.The damage to Vehicle 2 appeared to be consistent with impact from another motor vehicle. To immediate passenger side of the Vehicle 2 I observed Vehicle 3 described as a 2011 Toyota Rav4 color black (MA Reg 47XB99). Vehicle was facing in an easterly direction across the driveway. I observed the Vehicle 3 to have extensive damage to driver's side rear quarter panel, passenger side front quarter panel and front end. Also observed was damage to south east corner of the dwelling of 74 Eddy St. This area of the dwelling is located adjacent to the drive way. The damage appeared consistent with the impact caused by a motor vehicle. Also observed were large amounts of debris located in the area of the crash which included the sidewalk, yard and the driveway of 74 Eddy St. The debris included pieces of concrete which appeared to have been dislodged from the concrete light pole. A city of Newton no parking sign was lying in the yard of 74 Eddy St. This parking sign would have been attached to the concrete light pole due to ongoing road construction on Washington St. I also observed two tire marks on a curb stone located next the westbound lane of Washington St. These tire marks were in front of the residence of 74 Eddy St and were to the immediate left of a storm drain. The tire marks were consistent with being struck by a motor vehicle. In between the tire marks and just after the curb there was a hole in the ground with wires sticking out of it. This hole appears to be the location the concrete light pole was located prior to impact. Tire marks were located on Washington St traveling from the east bound lane easterly across the westbound lane and ending at the tire marks on the curb. The total length of the tire marks was approximately 190 feet. After conducting an on scene investigation it appears that Vehicle 1 from the start of the tire mark in eastbound travel lane of Washington St traveled 190 feet in the easterly direct across the westbound lane of Washington St and impacted the curb in front of 74 Eddy St. The tire marks had lateral striations almost the entire length indicating that the front tires were rolling and sliding with little to no braking prior to impact with the curb. After impact with the curb Vehicle 1 exited the roadway, struck a concrete light pole. Vehicle 1 then traveled over a sidewalk, across the yard of 74 Eddy St and into the driveway of 74 Eddy St. Located in the driveway of 74 Eddy St were Vehicle 2 and Vehicle 3. Vehicle 2 was parked facing toward the rear of 74 Eddy St. Vehicle 3 located to the passenger side of Vehicle 2 and was parked facing towards Washington St. When Vehicle 1 entered the driveway it struck Vehicle 2. The force of the impact of Vehicle 1 striking Vehicle 2 caused Vehicle 2 to strike Vehicle 3. The force of the impact of Vehicle 2 striking Vehicle 3 caused Vehicle 3 to strike the southeast corner of the dwelling located at 74 Elliot St. Vehicle 1 continued across the drive way and struck a white fence before coming to final rest. Vehicle traveled a approximately 55 feet from when it initially struck to the curb till final rest. No other injuries were reported on scene. All involved vehicles were towed from the scene by Tody's Towing. After the investigation the operator of Vehicle 1 Kyle Butler was issued Citations T2015882 and T2015883 for: Mgl 90/24/O Recklass Operation of a Motor Vehicle Mgl 89/4B/A Failure to Operate in the Right Lane Mgl 90/20 Failure to Submit a Motor Vehicle for Inspection Mgl 90/7 Defective Equipment (worn front tire treads) Mgl 90/13A Failure to wear a seatbelt An immediate threat form was also filed with the MA RMV. Operator of the black toyota, identified as Charlie Rodriguez reports driving S/B on Craft St when vehicle #2 who was behind him tailgating pulled around his vehicle then stopped in front of him causing him to crash into the rear of his vehicle. Operator of the blue mercedes, identified as Mario Santi reports driving S/B on Craft St and the cars in front of him were going to slow. He further reported that vehicle #1 stopped in front of him so he went around vehicle #1. Vehicle #1 then drove into his rear after he passed him.

On 12/05/2020 at approximately 22:00 hours I was called in to respond to a single vehicle crash with serious injuries in the area of 291 Nahanton St. Nahanton St is a pubic way in the City of Newton. Upon my arrival at the crash scene with Sgt Devine I was briefed by Officers Daly and Anderson, ( see Ofc Daly's report for further information CAD # 20054728 in regard to changes take out against the operator of the truck ) I made the following observations of the crash scene, Nahanton St is a public way in the City of Newton that is a two way roadway divided by an unprotected merdium ( double yellow line ) that runs east to west. The roadway surface was wet due snow and ice. The roadway was plowed and the snow was pushed to the shoulder, althought wet the roadway was icy. There was approximately 2 inches of snow on the shoulders of the roadway. The road was light with overhead head street lights. The posted speed limit is 25 MPH. I observed a 2013 Chevrolet Silverado pick up truck that had major front end damage. The Chevrolet was at an angle with it's front end facing west bound and resting on the grass shoulder and into a wooded area. The Chevrolet rear was sticking out into the shoulder of the east bound lane of Nahanton St. The passenger side front and rear doors of the Chevrolet had been removed by the fire department to extricate the driver and passenger. The driver and passenger side air bags were deployed . The front passenger side windshield of the Chevrolet was completely missing and the driver's side had completely shattered. The front tires, hood, and engine had been compacted on it's self. The front and rear driver's side doors had been mechanically open by the fire department. The speedometer needle had stopped at 54 MPH. The Chevrolet left the roadway and left tire marks in the snow in the eastbound bike lane of Nahanton St. At a 106.4 feet from the beginning of the tire marks the first point of impact was with two tree stumps that were 3.5 feet from the roadway, four feet tall and 3.5 feet in diameter. The tire marks continue in the east bound bike lane and exit the roadway and onto a dirt shoulder at 136.1 feet from the beginning of the tire marks. The truck continued to the right of a utility pole and then it's passenger side made contact with a tree at 170.8 feet from the begin of the tire marks. The tree was 12 feet from the bike lane approximately 5 feet in diameter. The impact with the tree caused the truck to turn so that the rear was facing north east on the dirt shoulder and bike lane. The final point of rest of the rear passenger side tire was 200 feet from the beginning of the tire marks. The truck's rear diver side tire final point of rest was 209 feet from the beginning of the tire marks. The front driver's side tire was compacted into the trucks engine and the passenger side front tire was located behind the tree approximately 20 feet from the roadway. Sgt Devine took photos of the crash area and of the truck and the disk was download by the NPD's IT Bureau. The truck was towed from the crash scene by Todys's Towing to their lot for storage, I fill out a towed motor vehicle form. There were no reported witness who observed the crash. The crash was reported by an unknown passerby. On 12/07/2020 Ofc Gaudet faxed a request for preservation of medical records, blood and urine samples of Schneider to Beth Israel Deconess Medical Center.

On 12/24/20, while assigned to N493, I, Officer Conary, responded to the area of Westwood Street for a report of a hit and run that just occurred. I was already in the area and did not observe any vehicle driving fast. Upon arrival, I met with Owner of MV1 who stated he was inside at 21 Westwood Street when he heard crash. He looked outside and saw a white sedan, possibly a BMW, quickly traveling Northbound on Westwood Street. MV1 was parked right outside.There was damage to the rear of MV1. I saw surveillance video from 21 Westwood Street, which showed a white sedan reversing down the street quickly and hitting MV1 then continue to drive away. MV1 did not require a tow. There was no reports of injuries. Following is an update by Officer M. Boudreau: At 12:07 On 12/25/20 I was dispatched to 95 Pearl Street because the vehicle involved in the crash had been found. MA Reg#1VYW99 a white BMW M3 was parked on Pearl Street and had damage to the rear of the car. Based on the video I was shown the damage is consistent with the accident and it was the same type of car. The family of owner #1 asked around the neighborhood and learned that Qiushi Wang had been driving the vehicle. I spoke to the owner of vehicle #2 who eventually admitted that his son had been driving vehicle on Learners Permit. The father Ningshan Wang stated that he was also in the vehicle as a Licensed Operator. Qiushi Wang stated he thought he hit a snow bank, then stated maybe he wasn't driving at the time, he eventually admitted to hitting the parked car. Qiushi also was shown video and admitted it was his car in the video. Asked video be sent to my email or brought into NPD Both father and son will be issued citations for leaving the scene of property damage. On 1/21/21 I was working N493 when I responded to the area 450 Lexington St for a report of damage to city property from an MVA. Upon arrival I observed a parking sign that had been broken in half, Parking Meters that had been ripped from the ground and completely smashed, and 1 MA reg plate 8AH567. The plate came back to 398 Wolcott St. where I spoke to Dylan Crook. Crook stated he had been driving MA reg 8ah567 when he crashed and then went home. Crook stated he was distracted and reaching down to get something when he went off the road. Crook was given Ma unifrom citation T2080020 for 90/24/C Leaving the scene of an accident with property damage, which was explained to him. The city responded to clean up debris. Operator of MV1, Emily Stoller, states she was operating MV1 in the parking lot at 200 Boylston Street when her friend, Alvin Day, asked her if he could drive MV1, which she gave him permission to do and was under the impression Mr. Day had a valid driver's license. It should be noted this area is regularly used to get to commercial businesses and has the right of access to the public. Ms. Stoller states Mr. Day was only driving about 15mph when he lost control of the vehicle and crashed head on into a light pole. MV1 sustained heavy front end damage and was towed from the scene by Tody's towing. Mr. Day states he does not have a valid driver's license and asked Ms. Stoller if he could drive MV1. Mr. Day states he was "goofing off" driving around the parking lot at approximately 15mph when he "cut the steering wheel too much", lost control of the vehicle and crashed MV1 head on into the light pole causing the driver's side airbag to deploy. Mr. Day states he lost consciousness for a brief moment where he was assisted out of the vehicle by the rear passenger, Alex Kakooza. Mr. Day was evaluated by the medics and transported to Children's Hospital. Ms. Stoller and Mr. Kakooza were also evaluated by the medics where they obtained patient refusals. All three juveniles had an adult family member respond to the scene. Ms. Stoller's father, Gregory Stoller, Mr. Day's mother, Quonekuia Day and Mr. Kakooza's sister, Latifah Kakooza. Professional electrical contractors responded for the damaged light pole and mall security will be notifying the property owner.

On February 20th, 2021 at approximately 09:04 hours while working N491 I responded to a report of a past hit&run MV crash that involved two US postal mailboxes that had been struck near the intersection of Charlesbank Rd @ St James Terrace. On my arrival to the scene I located the first mailbox on the sidewalk near #93 Charlesbank Rd. The other mailbox was located on the property to #93 Charlesbank Rd upside down leaning up against the front of the house. Dispatch notified the postal authorities and they were going to respond to pick up their mailboxes. The vehicle left scene heading W/B on Charlesbank Rd. #94 Charlesbank heard a loud noise around 01:15 this morning but thought it eas just a plow.

The operator of MV#1 stated she was in the parking lot of 330 Homer St (Newton Free Library) attempting to park into a parking spot when her vehicle all of the sudden accelerated, went over the burm striking MV#2 (Parked and unoccupied), striking MV#3 (Parked unoccupied) and pushing MV#3 into MV#4 (Parked and occupied with 4 people). The operator of MV#1 stated she was not distracted and her vehicle just accelerated forward by itself. The front passenger of MV#1 stated they were attempting to park into a parking spot when the vehicle accelerated on its own. MV#1 sustained moderate damages to its front end. There were no reported injuries to the operator of MV#1 or its passenger. MV#1 was deemed mechanically sound and left the scene after being provided with the necessary information pretraining to this accident. MV#2 was parked next to MV#3 and was unoccupied when it was struck by MV#1. MV#2 sustained minor damages to its front passenger's side bumper. The owner of this vehicle was provided with the necessary information pertaining to this accident. MV#3 was parked in between MV#2 and MV#4 and was unoccupied. MV#3 was struck by MV#1 which pushed MV#3 into MV#4. MV#3 sustained heavy front end damages (Broken driver's side axle). The operator of MV#4 was inside his vehicle along with 3 other passengers when they were struck by MV#3. MV#4 sustained heavy damages to its driver's side rear door. There were no reported injuries to the operator of MV#4 or its 3 passengers. The witness stated he was inside his vehicle a few parking spots over from MV#4 and observed MV#1 drive over the burm striking MV#2 then striking MV#3 and pushing MV#3 into MV#4. MV#3 sustained the heaviest damage and was inoperable. MV#3 was towed by AAA Towing Services. I took digital photographs of the damages sustained to the city's burm.

I SPOKE TO THE OWNER OF 112 WALTHAM ST ( BRIGITTE LAMARCHE, 112 WALTHAM ST., NEWTON, MA., 02465, DOB: 3-7-71 ). OWNER STATES SHE WAS IN THE REAR OF THE HOUSE WHEN SHE HEARD THE CRASH AND THEN CAME AND FOUND VEHICLE #1 IN HER YARD AFTER DOING DAMAGE TO HER FRONT YARD. IT IS UNKNOWN AT THIS TIME IF THE VEHICLES IN THE DRIVEWAY HAD DAMAGE FROM THE CRASH DUE TO MUD AND GRASS ALL OVER THE VEHICLES. OWNER OF VEHICLE #1 REPORTED NO INJURIES. VEHICLE #1 WAS TOWED BY TODYS. AS A RESULT OF THE CRASH I HAVE CHARGED THE OPERATOR OF VEHICLE #1 WITH ( T2080806, CRIMINAL APPLICATION, 90/24 DRIVING RECKLESSLY, 90/17 SPEED GREATER THAN REASONABLE, 90/23 ATTACHING PLATES ). PICTURES OF DAMAGE AND SCENE WERE TAKEN BY SGT. FLEMING AND SENT TO I.T. BOTH PARTIES WERE ADVISED TO CONTACT THEIR INSURANCE COMPANIES. CLEARED WITHOUT FURTHER INCIDENT.

During questioning, Operator of MV1 admitted to consuming alcohol, appeared impaired and under the influence of alcohol on scene. Operator of MV1 completely left his lane of travel and was operating in a negligent manner. Operator of MV1 sustained injuries to his head and left ankle. He was then transported to Newton Wellesley Hospital for evaluation. MV1 sustained heavy front end damage due to impact. MV1 was unable to be towed due to live power lines underneath the vehicle. A voicemail was left with the traffic bureau to follow up on when it was safe to tow the vehicle involved. Operator of MV1 was mailed a criminal application, citation #T2012668 for the following offenses: 1. Ch. 90 Sec. 24/C (Leaving the scene of property damage). 2. Ch. 90 Sec. 24/E (MV, Negligent operation). 3. Ch. 90 Sec 24/J (OUI, Liquor). See incident #21016286 for further review.

On Sunday 4/11/2021, at approx 1219hrs, while assigned to N498, I responded to 141 Needham St in Newton for a MVA car into a building. There I observed MV1 in contact with the building used by retail store TJ MAXX. The operator, Beverly Linsky, was already outside the vehicle. I assisted Linsky to a bench where she was evaluated by Fallon medics. Linsky refused medical treatment. She stated that she was "fine". It should be noted that despite the heavy front end damage to MV1 the airbag did not deploy. Linsky states that while attempting to park her vehicle she accidently stepped onto the gas pedal instead of the brakes and drove into the TJ MAXX store building. MV1 was towed from the scene by Tody's towing service. The store manager was notified. Due to the statements made by the operator and witness, I will be submitting an Immediate Threat form to the RMV. See incident report #21015576 Pictures of the accident were submitted to IT.

Narrative:
On Monday May 10th, 2021 at approximately 0825 hours, dispatch sent units to the intersection of Eldredge Street and Vernon Street for a report of a road rage incident involving a crossing guard and a motor vehicle. When units arrived it was determined that the crossing guard who was performing her duties at the time of the incident, was struck by a motor vehicle that left the scene without stopping. I responded to the scene of the accident to conduct an investigation. At the time of the incident the weather was cloudy and the road surface was dry. Eldredge Street and Vernon Street are both public ways owned and maintained by the City of Newton. Upon arrival I spoke with the injured crossing guard inside the ambulance identified as, Margaret Monbouquette. Margaret was being treated by Fallon Medic 3 for leg pain. Margaret stated she was in the middle of the intersection holding her stop sign up as children and adults were actively crossing to get to the Underwood School. Margaret stated she had all three streets stopped when her attention was brought to the vehicle at the stop sign on Eldredge Street on the one way side. Margaret stated it kept inching forward into the intersection attempting to turn left. Margaret stated she still had people crossing at that intersection so she stepped in front of the vehicle and held up the stop sign directly in front of the vehicle and the female driver. Margaret stated she looked at the female driver and stated "No, stop" and that is when the driver started to yell back at her inside her vehicle but she could not make out what she was saying as her window was rolled up. Margaret stated the driver then tried to drive around her and struck her with the front passenger side bumper of the vehicle. She stated she was struck in her hip area which caused her to fall onto the street. Margaret stated the vehicle never stopped and sped up Vernon Street. Margaret described the driver as a slight female with dark hair driving a dark colored SUV. Margaret was able to provide a license plate of MA REG: 1XCS97. A query of the license plate through dispatch was able to provide an address of 74 Charlesbank Rd, Newton. Officers checked this address but got no response at the door and the vehicle was not located. A witness to the incident identified as Alicia Matos left her phone number and I was able to make contact with her over the phone. She stated she was in her motor vehicle stopped at the stop sign on Eldredge St, directly across from the vehicle involved. Alicia stated she was stopped because the crossing guard was in the intersection holding up her stop sign and was actively crossing children and adults. Alicia states she noticed the vehicle across from her kept creeping out into the intersection trying to take the left turn up Vernon Street. She states the crossing guard walked over to that vehicle and told her to stop and saw the female driver start yelling and pointing her hands at her inside the vehicle. Alicia states the crossing guard then backed up and was looking down at the drivers license plate when the driver drove at the crossing guard and struck her with her front passenger side and knocked the crossing guard to the ground. Alicia states the vehicle clearly made contact and did not stop and left the scene turning onto Vernon Street at a high rate of speed. Alicia states several people in the area came to the crossing guards aid and police and medics arrived shortly. Alicia described the driver of the motor vehicle to be a female with dark hair. The principal of the Underwood School, Suzie Talukdar was present after the accident and stated the secretary of the school received a phone call shortly after the incident from a grandparent of the school. The grandparent was identified as Dawn Coleman. Dawn stated her daughter was driving the vehicle involved and was having anxiety over the incident and didn't know what to do. Dawn identified the driver as Chelsey Sullivan (S83874401). I attempted to make contact with Chelsey at her residence of 74 Charlesbank Rd with negative results. The secretary was able to provide phone numbers for Dawn and Chelsey and I responded to the police station to attempt to make contact. While at the police station, I was notified that Chelsey Sullivan was downstairs in the lobby. I met with Chelsey and she stated the following: She was driving MA REG 1XCS97 a black Toyota Rav4 and was stopped at the stop sign on Eldredge Street the one way side. She noticed the crossing guard in the intersection holding up the stop sign but stated there was no one crossing at the time. Chelsey stated she started to inch forward and wanted to turn left onto Vernon Street as she was in a rush. Chelsey stated the crossing guard came right up to her vehicle and started shouting and holding up the stop sign in her hand telling her to stop. Chelsey stated she was yelling back at her stating that the cars behind her were going so she wanted to go too. Chelsey stated when the crossing guard started to walk away from her, she got very anxious and decided to take the left onto Vernon Street. She stated she saw the crossing guard stumble and fall to the ground but at no time did she think she made contact with the crossing guard. Chelsey stated she drove home and called her mother to tell her about the incident. Chelsey stated she wrote an apology letter to the crossing guard and has really bad anxiety and wasn't sure what to do. I advised Chelsey she needs to follow the crossing guards instructions when they are performing their duties no matter if she thinks the road is clear for her to enter/pass. Chelsey stated she understood and was also advised she will be getting a citation and summons in the mail. Chelsey took me to her vehicle where I took several photographs and submitted them to the IT Bureau. No damage was reported or observed. Massachusetts Uniform Citation 997066AA and criminal application was issued and mailed to Chelsey for Chapter 90 section 24 Operating to Endanger and Chapter 90 section 14 Failure to slow near pedestrian.

Narrative:
Operator of MV#1 stated he was driving eastbound on Auburndale Ave when MV#2 came towards him. MV#2 swerved into the wrong lane and crashed into the driver side of MV#1. MV#1 attempted to pull up on the curb to avoid the crash and got a flat tire on the front passenger side. MV#1 sustained damage to the entire drivers side of the vehicle and had to be towed from the scene. MV#2 was described as a RAM pick up truck that fled the scene. I was able to view dash cam footage of the crash which confirmed the events as described. The truck did not have a front license plate and was not able to be identified. It sustained damage to the drivers side. Officers checked the surrounding area with negative results. David Donahue from Tody's towed the vehicle from the scene. The occupants were uninjured and took an Uber.

On 6/18/21 at approximately 0119 hrs Sgt. Fleming, Officer L. Keefe, and I, Officer Mildner responded to Sumner St @ Willow St. (public ways in Newton) for a report of a single motor vehicle crash. Upon arrival I noticed MA Reg: 8SP545 up on the sidewalk, air bags deployed, with heavy front end damage from what appeared to be from crashing into a tree. The operator was identified as Saurabh Korgaonkar, who was being evaluated by the medics and signed a patient refusal. I spoke with Korgaonkar and asked him what happened. As I began speaking to Korgaonkar I noticed he a strong odor of alcoholic beverage coming from his breath, glassy eyes, and slurred speech. Korgaonkar stated he was driving home from Brookline somewhere in the area of Commonwealth Ave. and Washington St. I asked Korgaonkar where he was specifically coming from and he stated "Abbey". I asked Korgaonkar if he had anything to drink while he was there to which he stated "two drinks". It should be noted Korgaonkar was unsteady on his feet and seemed extremely confused about what just happened. Given the damage from the crash, my conversation with Korgaonkar, and Korgaonkar's current state, I asked him if he was willing to perform Field Sobriety Tests. Korgaonkar stated he was willing to take the tests. I asked Korgaonkar if he wears glasses or contacts to which he stated "no". I asked Korgaonkar if he had any medical conditions that would affect his ability to take the tests to which he stated "no". I asked Korgaonkar if he felt comfortable in the shoes he was wearing to which he stated "yes". I brought Korgaonkar over to a clear spot on the side of the road on the fog line where the area was illuminated by the street light and cruiser spotlight. There were no blue lights on the cruisers. I used a field sobriety card I carry with me on my person to explain and instruct the tests. The first test I administered to Korgaonkar was Horizontal Gaze Nystagmus. I explained to Korgaonkar I was going to use a pen as the stimulus and to only follow it using his eyes only, not his head. Korgaonkar stated he understood the directions. The first attempt, Korgaonkar follow the pen using his head. I explained to Korgaonkar not to follow the pen using his head, but his eyes only. Korgaonkar stated he understood. The second attempt, Korgaonkar followed the pen using his head. I advised Korgaonkar again, to only use his eyes. On the third attempt, Korgaonkar followed the pen using his eyes only and I saw distinct nystagmus at maximum deviation and onset of nystagmus prior to 45 degrees. The second test I administered was the 9 Step Walk and Turn. I demonstrated the test for Korgaonkar and instructed him on how to complete the test. I placed my left foot on the fog line and right foot, heal to toe in front of the left. I demonstrated three heal to toe steps on the line, counting out loud, and keeping my hands by my side. Korgaonkar was advised to take 9 heal to toe steps, and then turn around and complete 9 more heal to toe steps. Korgaonkar stated he understood. During the test Korgaonkar did not place his heal to toe on steps 3,4,5. Korgaonkar almost fell over on the fourth step. Korgaonkar did not count out loud his steps and was using his arms for balance. On Korgaonkar's return, he did not count out loud again, almost fell to the side on step 6, was using his arms for balance, and stopped the test on his own at step 7. The third test I administered was the one leg stand. I explained to Korgaonkar the instructions and demonstrated the test for him. Korgaonkar stated he understood. I placed my hands by my side, feet together, raised my right foot six inches while looking at my foot, and counted 1001, 1002, 1003. I asked Korgaonkar if he understood, and not to stop counting until I told him to. Korgaonkar stated he understood. On the first attempt, I told Korgaonkar to start and he didn't lift a foot off the ground. On the second attempt, Korgaonkar lifted his left leg off the ground two inches, was looking straight ahead, and was not pointing his left foot. I explained to Korgaonkar his foot needed to be six inches off the ground, pointed, while keeping his eyes on his foot. On the third attempt, Korgaonkar dropped his left foot on 1004 and lost his balance on 1012. I concluded the test at 1020 counts. Based on my observations and Korgaonkar's performance on the field sobriety tests, I concluded that Korgaonkar had been operating a motor under the influence of alcohol. Korgaonkar was handcuffed (DL), searched, and placed in the rear of 496 where he was seat belted. The prisoner compartment was checked prior to and after transportation to NPD. Korgaonkar was booked by Sgt. Fleming and processed by Officer Ka. Donahue in the normal fashion. During booking, Korgaonkar was read his rights and asked if he wanted to take a breathalyzer test. Korgaonkar refused the breathalyzer test. Korgaonkar was notified his license was suspended, and his MA drivers licensed seized. Officer L. Keefe stood by and waited for Tody's to arrive and tow away the vehicle. A towed MV form was filled out and placed in appropriate mailbox. Korgaonkar was charged with the following: MGL Ch. 90 Sec. 24/J OUI Alcohol (Korgaonkar crashed his vehicle and failed field sobriety tests) MGL Ch. 90 Sec. 24/E Negligent Operation of MV (Korgaonkar failed to operate his vehicle in a safe manner) MGL Ch. 89 Sec. 4/A Marked Lanes Violation (Korgaonkar crossed the fog line, went onto the sidewalk, and hit a tree) Photo's of scene sent to IT. A court packet was filled out with all pertinent information and placed in the court prosecutors mailbox.

On Thursday 7/15/21 at approximately 0900hrs I was dispatched to 12 Rockland Street for the report of a motor vehicle with heavy damage resting in the middle of the street. Upon arrival I observed a brown Nissan Altima unoccupied not running in the middle of street. Heavy front end damage on the passenger side along with two flat tires on the passenger side. There was no operator on scene but the car was registered to 12 Rockland Street. It should be noted that Rockland Street is a public way located in the City of Newton. I spoke to the reporting party Barry Cocuzzo who lives at 18 Rockland Street. Mr. Cocuzzo stated he witnessed William Power operating the Nissan and turn onto Rockland Street. Mr. Cocuzzo observed Power drive into his driveway with the two flat tires and heavy front end damage. When Power exited the vehicle Cocuzzo yelled over to him but stated Power's was unable to communicate clearly to him. Cocuzzo observed Power's walk into his residence at 12 Rockland and then bereaved the vehicle slowly roll back out of the driveway. Medic 3 and Tody's responded to the scene. Due to the heavy damage and two flat tires Tody towed the vehicle. A towed motor vehicle inventory report was filled out. Medic 3 was called to the scene to evaluate the health of Power's. Officer Tocci and myself located Power's in the residence passed out on the couch. Empty alcohol cans were all around Power's. After several attempts to wake up Power's he finally came too. Power's speech was severely slurred, he could barely keep his eyes upon and he lacked the ability to stand up without assistance. The Medics transported Power's to Newton Wellesley. Power's is being cited (Ma Citation 069847AB and 06850AB) for 90/10/A Unlicensed Operation, 90/24/E Negligent Operation of MV, 90/24/C Leave Scene of Property Damage, 90/24/J OUI Liquor and 90/24/P Use MV Without Authority. 5 photos were taken of the motor vehicle. It should be noted that the original location of the crash and the object that was struck are unknown to us at this time. Dispatch was also able to confirm that Power's did not have an active license at this time. Power's mother Bernice Fougere who is the owner of the vehicle stated to me that Power's did not have permission to use her vehicle.

MV1 described as dark gray pick up truck was traveling Eastbound on Franklin St. MV2 (MA Reg: 1XEF21) was traveling Northbound on Waverley Ave. MV1 crashed into MV2 driver side back passenger door, causing MV2 to spin 90 degrees and crash into a traffic light at the Northeast corner of Waverley Ave and Franklin St causing damage to the trunk and damage to the Traffic light owned by the City of Newton (5 photos taken, placed in IT). MV1 did not stop and continued to Eastbound on Franklin St. The operator of MV2 (Jessica CARNEY) and her passenger ( Madeline KLIMM) both signed patient refusals with Fallon ambulance. MV2 was towed by Tody's towing. There was a plate baring MA Registration : 3PMK11 left behind on scene which comes back to 2005 Ford Explorer, registered to a Berhe DANIEL. Berhe DANIEL is being issued a criminal complaint (Citation# T1445433) for MGL Ch. 90 s.24- Leaving the scene- property damage. Contacted Malden PD to check for vehicle. No car was at the house that the plate was registered. A witness ( Kathleen MORRISON), who was no longer on scene stated the plate of the vehicle that took off was MA Registration: 3PMK11. A Ford symbol and a Chevy hub cap were found at scene of accident. Due to this and witness description it is unclear if Plate of vehicle number 1 was attached to the registered car.

On Thursday 7/22/2021 at approx 1358hrs, while assigned to N494, I responded to the intersection of Newtonville Ave and Centre St in Newton for a 2 car MVA. Operator of MV1 states that he was traveling NB on Centre St and activated his right turn blinker to pull into the driveway of 570 Centre St where he was doing chimney work. He said as he began to turn, MV2 attempted to pass him on his right. They collided and MV1 sustained damage to the passenger side of his van near the front wheel. Operator of MV2 states he was traveling NB on Centre St when he observed MV1 in front of him stop near the middle of the intersection at Newtonville Ave and Centre St. He said he thought MV1 was taking a left turn so he attempted to pass him on the right. It should be noted that Centre St at this location has is only one lane both ways..There were no injuries due to this accident and neither was towed.

Narrative:
On 07/28/21 at 13:39 hours, I (Traffic Unit # 523) responded to 10 Cummings Road for a past hit and run motor vehicle bicyclist crash which occurred approximately twenty minutes prior. Dispatch updated units that the accident occurred at the intersection of Watertown and Walnut Streets and that a white colored SUV left the scene after striking the bicyclist. The white SUV was last seen heading southbound on Walnut Street towards Washington Street. The bicyclist then walked back home to 10 Cummings Road and told her mother about the incident. The mother then notified NPD. Watertown Street and Walnut Street are both public ways maintained by the City of Newton. It is a four way intersection with crosswalks on every side. There are no turn on red signs on every corner of the intersection. The weather at the time was partly cloudy and sunny and the road conditions were dry. On arrival at 10 Cummings Road, I spoke to the mother identified as Vanessa Cruz and her daughter, Angelique Cruz, who was involved in the crash. Fallon Ambulance was already on scene and a patient refusal was signed as Angelique did not have any injuries. I asked Angelique what had occurred and she stated that she left FA Day Middle School at approximately 13:00 hours. It is estimated that the crash took place between 13:10 and 13:20 hours. She was then bicycling down to the intersection of Walnut & Watertown Streets. She says that she pushed the walk signal button to cross the street at the crosswalk to continue southbound on Walnut Street towards Lowell Avenue. Once the cross signal light activated, she bicycled through the crosswalk. At the tail end of the crosswalk, her rear bicycle tire on the right side was struck by a white SUV making a right turn on to Walnut Street from Watertown Street. She was then knocked to the ground where she sustained a scrape mark to her left knee on her sweat pants. The white SUV continued southbound on to Walnut Street without stopping. She feels that the driver might of misjudged taking the right turn and never realized that she struck her. Angelique says she only saw a female operator in the SUV and could not determine the race or age. She also did not observe the registration on the vehicle. She further stated that a female assisted her up from the ground. Once she said she was fine she then proceeded home. No one else phoned 911 regarding the incident. At this point, I asked to see the bicycle which was stored away. The bicycle is a Schwinn Sidewinder 21 speed color black with lime green block style writing for the brand and model on it. Both tires have white reflectors attached to them. I observed no damage to the bicycle. At the time of the crash, Angelique stated she was wearing purple sweat pants, blue sweater, and no bicycle helmet. She was also carrying a black backpack attached to her rear shoulders. I took digital photos of Angelique along with her Schwinn bicycle. After, I went over to the intersection of Watertown and Walnut Streets and took photos of the scene. A canvass was also conducted that produced negative results. I observed no surveillance cameras on the buildings or residences that are near the intersection. At 109 Walnut Street, I spoke to several tenants in the six unit apartment complex and they have no security cameras. 583 Watertown Street has no cameras along with 592 Watertown Street. I also checked near the intersection of Walnut and Washington Streets as the white SUV was last seen heading in that direction. The Trio apartment complex along with other businesses are lined against both streets and have outside security cameras. I spoke to the leasing office and they stated that the cameras do not provide any coverage of the street area.

Operator of MV1 stated that they were at the red light at Washington and Cherry St heading westbound. MV1 was in the left lane, and MV2 was on the right turn only lane. MV2 continued straight on Washington St instead of taking the right turn. MV2 merged onto MV1's lane. At the following light Washington St at Elm St the light turned green and the Operator of MV2 sped up and cut off MV1. The operator of MV1 stated she had to hit her brakes at attempt to avoid being hit. MV2 ended up striking the front passenger side of MV1. MV2 continued on Washington St then merging onto Mass Pike heading Eastbound. MV1 followed MV2 and was able to get MA Reg V71107 Safelite Van. MV2 failed to stop after MV1 attempted to flag the operator down. MV1 had moderate damage to the passenger door, bumper, grill, and hood. Operator of MV1 stated she was not injured and was requesting a private tow. Traffic Bureau update (Officer Gaudet): On Monday, August 2, 2021, I spoke with staff at the Newton Safelite location to try and obtain information on the operator of MV2. Staff from the Newton location stated the vehicle involved was actually out of their Framingham location and I was provided with contact information for their District Manager Mr. Brian Nickerson. Mr. Nickerson was said to be privy to the crash and was investigating it. I attempted to contact Mr. Nickerson with a negative result. I left a message for him to contact me regarding the incident. To be further investigated. On Monday, August 2, 2021, I spoke with Mr. Brian Nickerson who stated he was going to contact his fleet manager and try to get more information on the vehicle. Charges are still pending.

Upon arrivial, I observed Vehicle 1 crashed into a tree at the corner of Waltham St and Orchard Ave. The vehicle appeared to travel southbound on Waltham street and crossed the double yellow line and collided with a tree. I observed heavy front end damage to the vehicle and the tree. The vehicle was unoccupied, and the right passenger air bag had gone off. Two witnesses, Gonzalo Milet and Can Erbil both stated they heard the crash and saw two male parties running down Orchard Ave towards Adella Ave and Watertown St. While multiple other units were checking the area they located male parties, Jonathan Sajquin Carey and Cristian Vargars at the corner of Adella Ave and Watertown St. Both parties were unsteady on their feet. Mr. Sajquin Carey stated that he was the operator. He was issued Massachusetts Uniform citations T1445449 and T1445449. See incident report # 21036126. Vehicle 1 was towed by Tody's towing.

On Thursday 9/16/21 at approximately 1326hrs, I responded along with Officer Gabriel (495) to the area of Boylston Street and Sheldon Road for the report of a single motor vehicle accident. Upon arrival I observed Ma reg(9JV392) on the sidewalk stuck in a stop sign. The vehicle was unoccupied at the time with no operator in sight. The motor vehicle sustained heavy damage and was towed by Tody's. I notified dispatch to the damage to the Stop Sign and Hidden Driveway sign. Traffic Officer Wilson took photos of the accident scene. MV1 was traveling Westbound on Crafts St. when MV1 crossed the double yellow solid line, hopped the curb and crashed into a telephone pole just prior to the intersection of Crafts St. and California St. Owner/ operator of MV1 stated he was rear ended causing him to crash. Located in front of 324 Crafts St. There were no indications of a second vehicle and the operator was concluded to be under the influence of alcohol. See incident report 21039916 for further information. The vehicle had not been towed yet, since the vehicle was trapped under a broken/ hanging part of the telephone pole. The vehicle will be towed as soon as the power company is able to replace the pole.

On 9/22/21, Sgt Walton was in N506 (first half). H was attempting to stop a fleeing suspect after a shop lifting incident. MV Operator #1, Newton Police Sgt Walton attempted to block an egress from a parking lot located at #0 Dedham Street. This is the driveway to and from the Newton Highlands Playground. MV Operator 2, Demaris Hardy admitted that he was trying to flee when he made contact with the newton cruiser. On October 4th, 2021 at approximately 11:58 hours while working N491 I responded to a report of a past Hit & Run involving a parked MV in front #350 Watertown St, Colonial drug. On my arrival I located one of the involved vehicles a 2009 blue Toyota Venza MA plate RUBRIC with the owner, Suzanne McGowan inside it. She reported parking her car in front of #350 Watertown St while out doing errands. She came back a short time later and found her vehicle had been struck. She found a note attached to her vehicle that said," I'm sorry I hit your car, with a telephone # 617-448-1039," call me". I spoke with the R/P, witness JP Botindari, address Colonial drug, #350 Watertown St. He reported hearing a crash from inside his business which brought him outside to investigate. He then observed a blond haired white female, 40's ruddy complexion wearing ripped blue jeans put a note on the parked vehicle she just hit. He asked her if she was ok? Women said nothing before getting into a grey colored SUV Ma reg.888-oyd.with heavy passenger side damage. The vehicle fled E/B on Watertown St. The registration was later queried but didn't come back to any vehicle. The phone # left on the note was traced back to a women out of Arlington Ma. but after dispatch had spoke with her realized this was not the women involved. The only vehicle attached to her name was a Nissan Pathfinder. Her drivers license picture didn't match the witnessed description either. An attempt will be made to identify the other vehicle through similar license plates. Traffic Bureau update (Officer Gaudet): A query of MV2 (MA: 8880YD) showed it comes back to a 2016 gray Jeep Cherokee registered to Ms. Johanna McGlame out of Newton. I spoke with Ms. McGlame via telephone. Ms. McGlame stated he was involved in the crash and did leave a note on the vehicle she hit with her phone number. Ms. McGlame stated she was traveling on Watertown Street and the passenger side of her vehicle made contact with a parked car. Ms. McGlame stated her vehicle had damage as a result of the crash and it is currently being fixed. Ms. McGlame's vehicle information and contact information was updated in this report. Ms. McGlame will not be cited for leaving the scene of property damage.

On 10/7/21 at about 1145am I was dispatched to 1101 Beacon St the Walgreen's parking lot for an MVA with injuries. On arrival, I observed 3 parked unoccupied MVs heavily damaged that were smashed into each other. I was approached by one of the MV owners, Tony Dooley. Mr Dooley stated he was walking out of the Walgreen's front door heading to his parked MV (#3) which was approximately <30 feet away. His attention was suddenly drawn to a speeding MV (MA reg 3MKH84) driving through the lot that veered right and smashed into the 3 cars. Mr Dooley's MV was the middle car smashed between the other 2. The female operator briefly stopped afterwards and pulled into an adjacent parking spot at a crooked angle. I approached the female operator who was being taken out of her car and placed onto the stretcher by medics for transport. Female operator was identified as Ms Norma Greenberg. I asked her what happened? She replied in a bewildered manner "I have no idea." I asked her if she was taking any medications today and she replied yes, to treat a spinal condition she has. Ms Greenberg had lacerations on her legs and arm with heavy bleeding as a result of the impact.Ms Greenberg's vehicle (#1) was towed with significant front damage and airbag deployment. MV#2, the first car struck on impact, was towed with very heavy passenger side, rear end damage and driver's side damage. MV#3, the middle vehicle, was towed with significant passenger side damage, flat tire and driver's side damage. MV#4, sustained significant passenger side damage which was able to be driven from the scene. Based on observations and statements, it appeared Ms Greenberg had suddenly dangerously accelerated through a congested parking lot causing heavy damage. Ms Greenberg's driving history shows surchargable accidents from 7/1/21, 11/1/20, 8/25/15 and 12/26/10. Due to Ms Greenberg's inability to recall how or what happened, I am seeking an Immediate Threat application due to and not limited to medical reasons to suspend Ms Greenberg's license through the RMV.

MV1 and MV2 were involved in a road rage incident in the area of City Hall on Commonwealth Ave. Commonwealth Ave is a public way that is owned and maintained by the City of Newton. Stated by MV2 operator, James DAVIS: MV1 was operating his vehicle erratically behind and tailgating closely behind MV2. MV1 turned on his flashing white lights that were mounted in front of his driver and passenger side visors. MV2 believed that he was being pulled over by law enforcement so he turned right onto Chestnut St in order to pull over safely. MV2 operator, DAVIS observed the flashing light vehicle continue straight on Commonwealth Ave. MV2 continued his drive from Chestnut St to Temple St to Putnam St where he observed that same vehicle, MV1, driving around the rotary on Washington St over the Mass Pike heading in his direction. DAVIS stated that MV1 pinned MV2 by the driver's side door (B pillar). DAVIS was exiting his vehicle when MV1 closed in on his leg, injuring DAVIS with the car door and HELMS front bumper. Damage was visible on MV1 passenger side front bumper and MV2 driver's side door. DAVIS did not want medical treatment for his leg. Stated by operator HELMS in MV1: MV1 was being followed by MV2 in a dangerous manner in front of City Hall on Commonwealth Ave. MV2 operator, DAVIS, exited his vehicle when MV1 was stopped at the red light (Commonwealth Ave/Chestnut St). MV2 operator, DAVIS, threatened HELMS and brandished a knife. HELMS stated that he was scared for his life. At some point in the interaction, DAVIS got out of his vehicle again and brandished the knife again. HELMS was able to escape DAVIS and continue driving until he reached the backyard of the police station. During my interview with HELMS in the backyard I observed him trembling uncontrollably. I asked if he took anything tonight to which he said no, he was just cold. I asked if he smoked anything and he said he smoked a little marijuana earlier this afternoon but nothing since. HELMS is physically unable to complete field sobriety due to his past injuries including a TBI from a motorcycle accident, a rollover car accident and a bicycle accident where he was not wearing a helmet. He stated that he has multiple pins and plates in various parts of his body and does not feel safe doing field sobriety. I did not observe any odor of alcohol and HELMS stated that he has been sober for 10 years now. A MV search was conducted as well as pat frisk of DAVIS for officer safety. No knife was found on his person or in his vehicle. A MV search was conducted of HELMS vehicle due to arrest. Tody's towed HELMS' vehicle. Theodore Helms is being charged with the following: 90/7E - Improper Display of Flashing Lights (HELMS' mounted white light bar that flashes oscillating lights when turned on). 265/15A - Assault & Battery with a Dangerous Weapon (HELMS' vehicle pinning DAVIS' leg between his driver's side car door). UPDATE: 0030hrs DAVIS returned to the front desk of headquarters to claim that his leg was throbbing (the leg that was pinned between his car door by HELMS' vehicle). Pictures were taken of his knee and the SD card was sent to IT.

On 10/22/2021, at approx 0752Hrs, while assigned to 497, I responded to 2071 Washington St for a report of a past hit and run with property damage. Upon arrival I observed the tire tracks leading from the right westbound lane leading over the curb , knocking over a chain link fence and post . There were tire ruts into the side yard of 2071 Washington St, an there was a End of Reduced Salt sign and post wedged into the ground of the homeowners side yard. I spoke with the property owner , Ali Shamser who stated at approx midnight he heard a loud bang, and looked out his window , saw nothing in the road but did not think to look down into his side yard. Ali's home is across the street from Newton Wellesley Hospital and next to Route 128 as well as on a very busy road so he is used to hearing crashes and sirens regularly. Ali stated at approx 0745Hrs when he left his home to take his children to school he noticed his damaged fence and yard. Ali has no cameras, across the street is Newton Wellesley Hospital and his neighbor adjacent on Walsingham St has his property surrounded by a 6 foot vinyl fence.

On October 25th, 2021 at approximately 08:32 hours while working N491 I responded to a report of a MV crash in the area of Walnut St near Minot Place. On my arrival I located both involved vehicles near #100 Walnut St, The School Administration Building. Vehicle #1 was a blue 2015 Prius, Ma reg.945-AM3, operated by a William Aldridge. He stated he was first stopped N/B at the intersection of Walnut St @ Watertown St in the right turn only lane. The light turned green and realized he wanted to go straight so he continued heading N/B on Walnut St . However as he went straight he cut off vehicle #2 who was in the lane to his left who then honked his horn at him. Vehicle #1 then stopped ahead of vehicle #2 a short distance down the road. Vehicle #2 then drove around vehicle #1 who was still stopped. Vehicle #2 after going around vehicle #1 had to stop almost immediately due to a pedestrian crossing the street. Vehicle #1 at the same time proceeded forward behind vehicle #2 but then rear ended vehicle #2 after it stopped for the pedestrian. Vehicle #2 was a 2021 grey Toyota Highlander, Ma reg. 447AB8, operated by a Rizwan Khan. He stated he was stopped for the light N/B on Walnut St @ the intersection of Watertown St. He was in the lane for straight ahead traffic. As the light turned green vehicle #1 cut over into his lane ahead of him causing him to almost hit him. He then admitted beeping his horn at vehicle #1. Thats when vehicle #1 suddenly stopped in front of him. After vehicle #1 didn't move he decided to go around him. As he passed him there was a pedestrian crossing the street. He then stopped his vehicle so the person could cross. His vehicle was then rear ended by vehicle #1.

At approximately 0922HRs on Friday, October 29, 2021, I was dispatched to 265 Washington St. for a motor vehicle crash with minor injury. Upon arrival, I observed MA Pass: 1ZCM56 parked almost in the on-ramp to I90, causing a potential safety hazard for drivers attempting to enter/exit the highway. The vehicle's driver's side headlight was broken, its front bumper/driver's side fender was partially dislodged and there were scratches and dents in the general area of impact. Parked in front was MA Pass: 2CHV64, which displayed a large scrape on its passenger's side rear bumper, gouges missing from around the wheel well and scratches through the general area of impact. Both vehicles were owned by the operators. The first of which being operated by Marineyde Oliveira (OLN: no status/NRE) and the second of which being operated by Brittany Rivera (MA OLN: S95463618). With the translation assistance of Officer Calderon, I spoke with Marineyde, who stated she was traveling westbound on Washington St., by the entrance to I90. At this time, she attempted to change one lane to the left, when she struck Brittany's vehicle in the rear. Upon requesting her driver's license, Marineyde stated she did not have one.. Upon speaking to Brittany, she stated she was traveling westbound on Washington St., in the lane closest to the concrete island, just before the entrance of I90. It was at this time that Marineyde entered her lane, striking her vehicle in the rear. Brittany stated she then pulled over and exited her vehicle, past the on-ramp and it appeared Marineyde initially was going to drive away without stopping. Brittany then began to signal her to stop by waving both of her arms and Marineyde parked behind her. Brittany, a Spanish speaker herself, asked Marineyde for her driver's license in Spanish, after she watched Marineyde exit the driver's seat, but Brittany was told she did not have one. Because Brittany was concerned that Marineyde initially appeared as if she was going to hit-and-run and because Marineyde did not have a Driver's License, she elected to call The Newton Police. It should be noted that Brittany was complaining of neck pain and stated she would drive herself to Newton-Wellesley Hospital; she signed a patient refusal. Due to Marineyde admitting to me that she was driving without a license, Brittany witnessing said illegal operation, the vehicle being registered to Marineyde and no other party being in her vehicle, I initially placed her under arrest for violating one count of MGL Ch. 90 S. 10 (Operating With Out A License). She was handcuffed behind her back, double locked and placed in the cruiser for transport. Officer Calderon began to transport her to Newton Police Headquarters. However, after discussing the matter with Sergeant Poutas, we came to the belief that the "in the officer's presence" element of the crime would likely not be satisfied by the above, so Marineyde was released forthwith. She will be receiving MA Criminal Citation: T1445781 in the mail for the above offense, as she was already released from the station prior to my arrival. Officer Calderon explained everything pertaining to the initial arrest and that she will be receiving the criminal citation in the mail. This rotary/intersection is a particularly dangerous area, in which we respond to many crash reports. The area where MA Pass: 1ZCM56 was parked, was creating a potential hazard for motorists entering the highway, as well as the many motorists who often exit the ramp, where she was parked, to avoid accidentally entering the highway. Because of this, Todys Services removed the vehicle. Note: Washington St. is A Public Way in The City of Newton. I completed and filed an NPD Towed Motor Vehicle Form and cleared without further incident.

At approximately 0214 hours on November 6, 2021, Officer Guarino and I, Officer Joyce were dispatched to 219 Commonwealth Ave for a report of a hit and run. I spoke to the driver of MV1 who said he was sitting in the drivers seat of his car when the driver of MV2 struck his vehicle from behind. The driver of MV1 then got out of his vehicle and took a video of MV2 before it fled the scene. MV 2 took a right onto Manet Rd and crashed into a city pole and sign. Photographs were taken of the city property and submitted to IT to be attached to the report. The driver of MV1 emailed me a copy of the video he captured, which was forwarded to IT to be attached to this report. Officer Guarino and I noticed multiple pieces of MV2 on the ground around MV1. Other officers searched the area for MV2 but were unsuccessful.

On Friday 11/19/21 at approximately 0938 hours while assigned to marked unit n497 I was dispatched to the area of 1256 Commonwealth Ave for a report of a past Hit and Run. Upon arrival in front of 1256 and 1270 Commonwealth Ave across the street on the Westbound side I observed a concrete light post broken at the base and laying in the Grass median between Commonwealth Ave and the Carriage Rd. The light pole had an electric wire attached to it that connected at utility pole 102/180. On the grassy area was several pieces of debris from the vehicle causing the damage, including what appeared to be an empty steel liquid storage container, possibly gas. there was no evidence of any sort of spill so the container may have been empty upon impact. After canvassing the area for evidence related to the vehicle involved I spoke with the resident at 1270 Commonwealth Ave, BLAKELY Thomas. Thomas states he heard a loud crash at approximately 0300am, but wasn't sure what it was. I noticed cameras on his home and asked if we could view the video. Video showed the vehicle impacting the pole and spinning out and then continuing Westbound on Commonwealth Ave Thomas emailed the video to me and Ofc Drew Vello. After returning to Newton Police Headquarters Ofc. Vello and I viewed the video and at 0300 hours a Tow Truck is seen veering off the road travelling Westbound on Commonwealth Ave and striking the light pole. Tow Truck spins sideways in the grass, and then continues Westbound on Commonwealth Ave. As the truck gets back on Commonwealth Ave there is lettering visible on the drivers side door, but Ofc Vello and I were unable to make out what the lettering says. I also took 7 photos of the area of the crash and emailed them to Officer Vello to attach to this report. Eversource and the City were notified to attend to the light pole, wires and debris.

On November 21st, 2021 at approximately 07:56 hours while working N491 I along with Officer Mike Boudreau responded with NFD and Medic One to a report of a MV crash with injuries in the area of Centre St @ Franklin St. On my arrival the Medics were evaluating two adult males who standing outside one of the involved vehicle from this crash. There appeared to be several involved vehicles to this crash however two of them had been parked prior to the crash. The vehicle which the two unidentified males were standing by was MA reg. 9HY939, a 1988 Black Ford F150 PU, listed registered owner was a Peter J. Jara-Castro, #28 Ridge Ave, Lynn Ma. Mr Castro was one of the two males standing by that truck. The above vehicle appeared to have been travelling N/B on Centre St when it skidded off the road, jumping the curb striking a 1 foot slate retaining wall to the property to #573 Centre St. It continued onto that property driving over shrubs, small trees, then onto the driveway(same address) which is actually on Newtonville Ave, hitting the homeowners 2007 Nissan Versa, Ma reg. 6BH269, forcing it into more shrubbery. The Truck continued across Newtonville Ave where it struck another parked vehicle, a Grey 2019 Volkswagon, Ma reg. 1NFC27 on the rear passenger side pushing it forward. The truck finally came to a stop after driving into a wooden fence to the property of #559 Centre St.(Fence is on Newtonville Ave.) I spoke with the owner of the truck after he was evaluated by the Medics, who spoke limited English. The owner of the truck Peter Jara-Castro finally said he was driving the truck prior to the crash. His statement was he was travelling N/B down Centre St when another vehicle came out from Newtonville Ave causing him to slam on his brakes. Mr Castro doesn't remember much after that, possibly from hitting his head. At this time I couldn't determine who the actual driver was. Both Mr Castro and the passenger, sustained injuries but both declined to go to the hospital. Both were later picked up by an UBER driver. A Firefighter heading to work reported seeing the truck pass him on Centre St N/B at high rate of speed but never saw the driver or the actual crash. Both property owners that sustained damage were given Trucks insurance info. Pictures were taken by both property owners. Vehicle #1 and vehicle #2 were towed from scene. Both sustained heavy damage. Vehicle #3 sustained minor damage I was unable to make contact with that owner at this time. However later in the day I was able to make contact with owner of vehicle #3. She was given all vehicle #1 information along with his insurance. A citation for speeding was later issued after investigation was completed and witness statement. Citation#t1444076 was later sent VIA mail.

At approximately 1358HRs, on Friday, December 3, 2021, I was dispatched to 225 Boylston St. for a report of a two car Motor Vehicle Crash with no injuries. Upon arrival, I observed both vehicles and noted the following: MV1 sustained scratches to its front passenger's side bumper, fender and headlight area. MV2 sustained damage to its front driver's side door. I confirmed with both operators and neither reported any injury. Upon speaking with the operator of MV1, they stated that they took issue to the way the operator of MV2 was driving around a rotary in The Town Of Brookline (Newton St. and Lagrange St.). They then proceeded in the same direction as MV2 and when they both were in the parking lot of 225 Boylston St., they attempted to pull their vehicle along side MV2 to voice their disapproval. At this point, MV2 was making a left hand turn and MV1 collided with MV2. Upon speaking to the operator of MV2, they stated that the operator of MV1 became upset with them in the Brookline rotary. They then proceeded to 225 Boylston St. to make a return of an item they had purchased. It was when they attempted to turn MV2 left (southbound)/towards an open parking stall that MV1 proceeded from behind them and collided with their door. During my investigation, I ran all parties and the license plates to the respective vehicles they were operating. It was during this time that I discovered the operator of MV1's Driver's License is suspended and I confirmed this through dispatch. The operator of MV1 was able to contact a licensed driver to respond to the scene to give them a ride. Note: This Motor Vehicle Crash took place in the parking lot of 225 Boylston St., a shopping mall, in which all the patroness share a public right of access.

On 01/03/22 at approximately 1617 hours Officer Robert March and I, Officer Whitney Hyde, were assigned to N499 when we were dispatched to a 2 car MVA at 160 Needham St with no injuries. Both vehicles were waiting in the parking lot. On location I made contact with the operator of Vehicle 1, identified as Samantha LERNER. LERNER stated that the had been travelling southbound on Needham St and stopped in the center left turn lane to turn into the Farm Grill. When it was clear to turn, LERNER began to turn left when Vehicle 2 passed her on the left and made contact with the driver side front quarter panel of Vehicle 1. I observed scrapes and minor damage to Vehicle 1's front bumper and driver side front wheel rim. LERNER stated that the operator of Vehicle 2, later identified as Leanna LANDE, exited her vehicle at the time of the accident and exclaimed that her dog was dying and she was headed to the animal hospital down the road and they could exchange information there. LERNER proceeded to the animal hospital and was waiting for LANDE to come back out. I then entered the animal hospital and made contact with LANDE. LANDE was extremely upset as her 7 week old puppy had just died. LANDE stated that she had been in a panic because of her dying dog and passed LERNER when she shouldn't have, stating it was "stupid." Outside of the animal hospital I observed Vehicle 2 had minor damage along the length of the passenger side, including scrapes with white paint transfer. I issued LANDE MA Uniform Citation #T1448086 in hand for 89/4A Marked Lanes violation and explained the payment and appeal process. LANDE stated she understood. I provided LANDE and LERNER with the accident number and facilitated their exchange of information before clearing without further incident.

On Tuesday, February 15th, 2022 at approximately 1908 hours the operator of Vehicle 1 was travelling Westbound on Auburn St when it crossed over the center double yellow lines, crossed the opposing lane of traffic, and went off the roadway before crashing through the vinyl privacy fence at #106 Auburn St. Vehicle 1 came to rest after colliding with the North-East corner of the residence. The operator of Vehicle 1 then attempted to reverse away from the crash and backed into and destroying more of the residence's privacy fence. The operator was then confronted by the occupants of the residence, including the homeowner. Fire and EMS arrived on scene and conducted patient care on the operator and formed the opinion that she was intoxicated. I arrived and interviewed the operator who stated that she had previously taken oxycodone. The operator was then transported to Newton Wellesley Hospital. The vehicle was towed from the scene by Tody's. The operator was charged with the following violations: C90ss24 OUI- Drugs C90ss24 Neg Op C89ss4A Marked Lanes An incident report was completed for this crash (See Report #22005833).

At approximately 1239HRs on Thursday, 04/07/2022, I was dispatched to 111 Needham St. (McDonald's) for a report of a hit & run crash. Upon arrival, I spoke with the operator of MV1 (MA Pass: 49315), who stated that he was in the restaurant when an unidentified concerned citizen came in and showed them a picture of MV2, after it had struck MV1 and informed them that the operator of said vehicle left the scene without leaving their information. I observed that MV1 sustained damage to its driver's side rear quarter panel, just to the rear of the wheel well. The owner of MV2 (MA Passenger: 313FY9) could not be reached to ascertain their version of events. Newton Dispatched was able to find a phone number for the registered owner, left a message and at the time of writing this report, The Newton Police have not been contacted by them. Additionally, I went to the registered owner's address and no one would answer the door, nor the Ring Video Doorbell, which remotely connects those activating it, to the owner's phone.

On April 9th 2022, at approximately 0038hrs, while assigned to cruiser N494, I, Officer Dragone, along with Officer Stake (N491) and Sgt. Brooks (N505) responded to the area of 196 Tremont St (public way in the City of Newton) for a report of a motor vehicle accident. Dispatch reported that the reporting party's vehicle was hit while it was parked in front of his address, and the operator of the other vehicle exchanged partial information and then left the scene. Upon arrival, I observed a blue Honda Accord bearing Massachusetts Registration: 1TDF63 (Vehicle 1) parked on the side of the road, blocking the driveway to 196 Tremont St. The vehicle had heavy damage along the driver's side of the vehicle and back left tire was pushed into the wheel well. In front of 190 Tremont St, I also observed a black BMW 528 bearing Massachusetts Registration: 1FDB38 ( Vehicle 2) with heavy damage along the passenger side of the vehicle and the front right wheel was completely detached from the axel. All the airbags in the vehicle were deployed, and it appeared that vehicle 2 was traveling Eastbound on Tremont St when it crashed into vehicle 1 at an angle. It should be noted that there were multiple deep scratches/dents on the hood, roof, and drivers side panels of vehicle 2 that were not consistent with the crash that just occurred; which indicates the operator was in multiple accidents prior to this one. Pro-tech Towing was also on scene to tow vehicle 2. I then spoke to the owner of vehicle 1, Ryan Desrosiers, who stated that his vehicle was parked on the side of the road when he heard a crash outside at approximately 2345hrs. Mr. Desrosiers stated when he went outside, he discovered his vehicle pushed in front of his driveway, approximately 10-15 feet from where he parked it, and the operator of vehicle 2 standing outside. Mr. Desrosiers stated when he spoke to the operator, he smelled like alcohol and had slurred speech. The operator, later identified as, Benjamin Wadsworth, told Mr. Desrosiers that he was "coming from in town" and "probably should have taken a Lyft tonight." When Mr. Desrosiers tried to exchange information, Mr. Wadsworth took Mr. Desrosiers' phone number and then texted him a link to a generic RMV website. Mr. Desrosiers then said Mr. Wadsworth called a tow company for his vehicle and insisted on paying out of pocket tomorrow for the damage to Mr. Desrosiers' vehicle but told him if he wanted he could call his insurance company. According to Mr. Desrosiers, Mr. Wadsworth then walked Eastbound on Tremont St towards Brighton. It should be noted that Mr. Wadsworth did not provide his name, address, or insurance company to Mr. Desrosiers prior to leaving. I had dispatch run a query on vehicle 2 (MA Reg: 1FDB38) and it came back registered to Benjamin Wadsworth out of 13 Charmane Rd East Falmouth, Massachusetts. While speaking with Mr. Desrosiers, the driver of the tow truck called Mr. Wadsworth cell phone to notify him that he was not going to be towing his vehicle. I was then able to get on the phone and speak with him, and he identified himself as Benjamin Wadsworth and admitted to being involved in the accident that just occurred. When I asked Mr. Wadsworth his location because I was concerned for his well-being due to the severity of the crash and airbag deployment, he stated he was at his apartment in Brighton. When I asked for the address of his apartment, he refused to give it to me and stated he was fine. I stated to Mr. Wadsworth that because he only gave limited information and then left the area that he could be charged with Leaving the scene of a collision, to which he replied, "I am in law school you can see the books in the back of my car, you can't do that, you have my plate, he can look that information online." When asked about having an address out of Falmouth, Mr. Wadsworth stated he has two addresses because he attends school in the area. It should be noted that while speaking with Mr. Wadsworth on the phone, his speech was slurred, slow, and dilerbrate, consistent with being intoxicated. Tody's towing arrived on scene and took possession of Mr. Wadsworth's vehicle. I then tried a second time to speak with Mr. Wadsworth via a phone call to get him to agree to a well-being check. Mr. Wadsworth, again, refused to provide me with his address in Brighton, insisting he was fine. Although Mr. Wadsworth sounded intoxicated, he did not sound injured and was coherent when answering my questions. Due to Benjamin Wadsworth operating recklessly, knowingly getting into a collision causing property damage, not providing his name, not providing his address, and leaving the scene, I am issuing him Massachusetts Uniformed Citation T1445620 for the following: MGL. Ch. 90 S.24 (Leaving the scene of a collision after property damage) MGL. Ch. 90 S.24/E (Negligent operation of a motor vehicle) A Request for Immediate Threat License Suspension/Revocation form was filled out and faxed over to the RMV.

On 4/21/22, I was dispatched to the intersection of Crafts St./California St. for a motor vehicle collision involving a single vehicle into a tree. Upon arrival, I observed MV#1 had crashed into a tree in front of 315 Craft St. The operator stated the "road was slippery" and he veered off the roadway, hitting the curb, before traveling 20-25 yards on the sidewalk, hitting two tree's. MV#1 sustained heavy front end damage and air bag deployment consistent with colliding into a tree at a decent rate of speed. MV#1 was removed from the roadway by Tody's Tow Service. The operator of Mv#1 was transported to Newton-Wellesley Hospital with injuries. Charges pending at this time for OUI Liquor 2nd Offense, Marked Lanes Violation and Negligent Operation of a Motor Vehicle.

On 5/11/22 I responded to Commonwealth Ave. and Center St. for a 2 car accident. Upon arrival I spoke with the operator of MV1 who stated he was at fault for the accident. He stated that he was on the center lane of Comm. Ave. heading w/b when he noticed that he had to take a right turn. He stated that he checked his mirror and reversed to merge onto the right lane, and that is when he hit MV2 which was behind him. The operator of MV2 stated that he was driving e/b on the center lane when MV1 backed up and hit him causing moderate damage to the right front of the vehicle. On May 14th, 2022 at approximately 14:29 hours while assigned to N491 I responded to a MV crash near #347 Watertown St. On my arrival I located both involved vehicles on Watertown St near Tommy Doyles. Vehicle #1 was parked in front of #354 Watertown St, a 2021 Gray Chevy Trailblazer, MA reg. 831ZY8. Owner Elizabeth Colon. Ms Colon reported she parked her vehicle across the street from Tommy Doyles, #349 Watertown St when vehicle #2 struck her vehicle. Unidentified witness alerted Ms Colon that they observed vehicle #2 hit her vehicle. Vehicle #2 was a RED 2007 Toyota RAV4, MA plate2HRN56, operated by Ana Cardoso Pereira. She stated she was attempting to park behind vehicle #1 when she accidently hit her accelerator instead of her brake causing her vehicle to go forward into the rear drivers side to vehicle #1. Note Ms Colon translated the above statement due to Ms Pereira only speaking Spanish. There were no tows required and no injuries.

On June 2nd, 2022 at approximately 13:02 hours while assigned to N491 I responded to #17 Russell Rd for a past hit & run crash. On my arrival I spoke with the R/P, Jason Kenyon. He stated he found a note on his vehicle from his neighbor, Monique Ricard, #18 Russell Rd, stating she observed a dark blue vehicle, Ma plate 3XCB84, operated by a B/M crash into the front of his vehicle. Mr Kenyons vehicle was parked in his driveway facing out. Wit ness thought the operator was going to leave a note but never did. I queried the suspects vehicles registration and it came back on a 2020 Blue Toyota Corolla, to a Samiir Muhamed, #36 Oakcrest Rd, Hyde Park Ma. Dispatch attempted to locate a telephone listing for the owner to no avail. Witness further reported that the suspect vehicle originally came out W/B from Charlesbank Terrence and appeared to have hit his accelerator instead of his brake causing his vehicle to accelerate forward into the driveway of #17 Russell Rd crashing into vehicle #1. Vehicle#2's damage would be on the passenger side front/quarter. video of this crash was later sent to the Traffic bureau. This crash will be further investigated by the traffic bureau.

The operator of MV1 stated he was traveling on North Street, driving approximately 70 mph and a skunk ran across the street. The operator stated he swerved to avoid the skunk, hit the curb and lost control of the vehicle. He stated once the car rested against the garage to 105 North Street, he and the passenger got out of the vehicle. He stated the passenger told him to leave. The operator stated he walked down Farwell Street and after thinking about what happened he came back to the scene of the accident. The operator and passenger stated they were not injured. After speaking with the operator, I observed tire marks going South on North Street and followed them to the curb where I believe he struck which was located approximately 100 yards, in front of 129 North Street. From my observations, MV1 struck the curb located in front of 129 North Street. After striking the curb, MV1 spun sideways, continued traveling southbound, until the MV crossed into southbound lane, struck the curb and then the utility pole 14. After striking the curb the MV spun around facing north, struck the retaining wall, bushes and light pole, located in front of 107 North Street. After striking the light pole, the vehicle hit the garage door at 107 and 105 North Street, resting against 105 North Street. There was significant damage to the garage doors to 105 and 107 and damage to the support beam located between the residences. MV1 sustained major damage to the motor vehicle. Todys was notified for a tow. Photos were taken of all damage and submitted to IT

On Tuesday, June 28, 2022, while assigned to unit N496, I responded to the area of Langley Road and Hamlet Street, Newton for a report of a roll over crash with 3 injured parties. The weather at the time of this crash was clear and sunny. The road surface in the area of the crash was dry. Langley Road and Hamlet Street are both public ways maintained by the City of Newton. I approached the area of the crash coming from Boylston Street and when I arrived my attention was drawn to two locations on Langley Road. I observed a female party laying prone on the sidewalk area of Langley Road (E) at the Hamlet Street intersection. This female party, later identified as Ms. Melinda Conroy, was being attended to by passerby's in the area. My attention was then drawn to an overturned vehicle resting half in the roadway/half on the sidewalk (W) in front of 323 Langley Road. As I began to run over to the vehicle, I observed a woman standing next to the vehicle screaming frantically. This party identified herself as the driver of the lone vehicle involved, Ms. Natalya Kabanovsky (S65512051). There were parties at the back of the vehicle yelling to me that there was a woman trapped under the vehicle and that they were holding the vehicle up to get her out. I ran over to the rear of the vehicle and observed a female party, later identified as Ms. Annette Sooper, lying underneath the vehicle in the roadway. There was a large pool of blood on the roadway as the result of a laceration to what appeared to be her head/face area. Due to the unknown stability of the vehicle resting on its roof, and due to the passerby's stating they were holding the vehicle up so it did not roll over onto Ms. Sooper, I reached under the vehicle from the rear, grabbed Ms. Sooper's person, and dragged her away from the vehicle to a safe location on the sidewalk. At this time, I observed a large laceration to Ms. Sooper's head and she stated she had suffered a leg injury. Newton Fire and Medics along with additional Police units arrived on scene and took over rendering aid to the victims. I then started to conduct my investigation into the crash. I spoke with the operator of the vehicle, Ms. Kabanovsky. Ms. Kabanovsky stated she was operating her 2005 Lexus ES330 (MA: 293TAB) on Hamlet Street (W) towards Langley Road. Ms. Kabanovsky stated as she approached the intersection, a female party (Ms. Conroy) ahead of her was running across Langley Road (S) at Hamlet Street. Ms. Kabanovsky stated she attempted to hit her brakes, but instead hit her gas pedal. Ms. Kabanovsky's vehicle crashed into Ms. Conroy and then she panicked. Ms. Kabanovsky stated her car then accelerated across the street and crashed. Ms. Kabanovsky stated she was not wearing a seatbelt at the time of the crash and extricated herself from her vehicle after it rolled over onto its roof. I requested Ms. Kabonovsky's license while speaking to her, but she was too upset to located it. I queried Ms. Kabanovsky's license and registration status through Newton Dispatch. Ms. Kabanovsky's license stated is expired as of 12/18/2016. Ms. Kabanovsky registration is active. I observed substantial damage to the entire front end, driver, and passenger side of Ms. Kabanovsky vehicle. Ms. Kabanovsky was evaluated by Newton Medics and complained of right arm pain. Ms. Kabanovsky was also visibly distraught and expressed concern over the two pedestrians involved in the crash. Ms. Kabanovsky was transported to Brigham and Women's Hospital for treatment. I briefly spoke with Ms. Melinda Conroy as she was being attended to by Newton Medics and Fire. Ms. Conroy stated she was running on Langley Road (S) and observed MV1 traveling on Hamlet Street (W) towards Langley Road. Ms. Conroy stated she believed MV1 was slowing down and coming to a stop. Ms. Conroy stated she waved at the vehicle and as she crossed the intersection, MV1 accelerated and crashed into her left side. Ms. Conroy stated she flipped over the vehicle and landed on the roadway. Ms. Conroy was conscious and alert when speaking to Newton Medics and was transported to Beth Israel Deaconess Hospital in Boston. I later spoke with Ms. Conroy after her release from the hospital. Ms. Conroy stated she suffered some bumps and bruises had no major injuries. Officer Pohlman spoke with Ms. Sooper prior to her transport to Beth Israel Deaconess Hospital in Boston by Newton Medics. Ms. Sooper stated she was walking on the sidewalk in front of 323 Langley Road after returning from Newton Centre. Ms. Sooper stated as she was walking, she observed MV1 hit Ms. Conroy and then travel across the street in front of her into a tree. Ms. Sooper stated she does not remember what happened after that. I spoke with Ms. Sooper's son at NPD HQ and was advised Ms. Sooper was admitted to the hospital and suffered a fracture of her left hip and required 6 staples to close a laceration on her head. Tody's Towing responded to the scene and removed MV1 from the roadway. Traffic Officer Wilson took pictures of the scene and submitted them to the IT Bureau. After speaking with all parties involved, Ms. Kabanovsky will be cited with Massachusetts Uniform Citation 494688AB in hand at her residence for Chapter 90, Section 10 (Unlicensed Operation), Chapter 90, Section 24 (Operate To Endanger), and Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Ms Kabanovsky was also cited with Massachusetts Uniform Citation 494800AB in hand at her residence for Chapter 90, Section 13A (Fail To Wear A Seat Belt). A Request for Immediate Threat License Suspension/Revocation was completed and submitted to the Registry of Motor Vehicles with supporting documentation via e-mail, DCUImmediateThreat@massmail.state.ma.us.
