The roadway was wet from previous snow fall but was very well plowed and sanded. MV#1 hit a curb, drove through a snowbank, hit a tree with the left front of his vehicle and then hit a pole with the right front side of his vehicle. The car then came to a rest in the median. The operator appeared confused by the accident and did not realize the extent of the damage to his car.He asked what he had hit. He continuously asked if it was driveable even though the entire front end was totaled. He was able to provide me with his license and all pertinent information but spoke in a slow manner. There was no odor of alcohol. Operator declined medical attention. When asked if he was on any medication he stated he was on heart medication. We were able to contact the operator's son who was transported to the scene and stated the operator had previously suffered a stroke. David Donahue arrived on scene towed the vehicle to Tody's. A licensed driver picked up the operator. A search of the immediate area yielded negative results for ice in the roadway. A request for Immediate Threat License Suspension/Revocation was filed.

Narrative:
On 12/05/2020 at approximately 22:00 hours I was called in to respond to a single vehicle crash with serious injuries in the area of 291 Nahanton St. Nahanton St is a pubic way in the City of Newton. Upon my arrival at the crash scene with Sgt Devine I was briefed by Officers Daly and Anderson, ( see Ofc Daly's report for further information CAD # 20054728 in regard to changes take out against the operator of the truck ) I made the following observations of the crash scene, Nahanton St is a public way in the City of Newton that is a two way roadway divided by an unprotected merdium ( double yellow line ) that runs east to west. The roadway surface was wet due snow and ice. The roadway was plowed and the snow was pushed to the shoulder, althought wet the roadway was icy. There was approximately 2 inches of snow on the shoulders of the roadway. The road was light with overhead head street lights. The posted speed limit is 25 MPH. I observed a 2013 Chevrolet Silverado pick up truck that had major front end damage. The Chevrolet was at an angle with it's front end facing west bound and resting on the grass shoulder and into a wooded area. The Chevrolet rear was sticking out into the shoulder of the east bound lane of Nahanton St. The passenger side front and rear doors of the Chevrolet had been removed by the fire department to extricate the driver and passenger. The driver and passenger side air bags were deployed . The front passenger side windshield of the Chevrolet was completely missing and the driver's side had completely shattered. The front tires, hood, and engine had been compacted on it's self. The front and rear driver's side doors had been mechanically open by the fire department. The speedometer needle had stopped at 54 MPH. The Chevrolet left the roadway and left tire marks in the snow in the eastbound bike lane of Nahanton St. At a 106.4 feet from the beginning of the tire marks the first point of impact was with two tree stumps that were 3.5 feet from the roadway, four feet tall and 3.5 feet in diameter. The tire marks continue in the east bound bike lane and exit the roadway and onto a dirt shoulder at 136.1 feet from the beginning of the tire marks. The truck continued to the right of a utility pole and then it's passenger side made contact with a tree at 170.8 feet from the begin of the tire marks. On 12/17/20 I responded to #964 Boylston St- The B.P. Gas station for an accident that happened this morning at 02:35. The owner of the gas station Elias Ghazale showed me video that showed a white pick-up truck with a plow that pushed snow across Ramsdell St onto a customers car. When the truck pushed the first pass of snow, the security camera shows the entire vehicle move violently into the curb. It is unknown at this time if there is any damage to the customers vehicle. It is also unknown at this time who was operating the plow or the name of the company. Because of all the snow plowed up on the customers vehicle, the license plate and information on the vehicle is also unknown at this time. I traveled to Negoshians Towing and left a message asking the owner to call me at the station to follow up on this matter. On 12/18/20 I spoke with Elias Ghazale who gave me a Mass. Registration of 1WPG55 a 2009 Honda Odyssey grey van that is owned by a Sarkis E. Ghazale as the owner of the parked vehicle that was involved in this accident. I traveled to Ferguson Plumbing and spoke with the manager Eric Mackenzie who stated the landlord, Negoshians Towing deals with the snow removal for his property. I then traveled to Negoshians and spoke with the Owner Ted Negoshian. He stated his company is responsible for plowing the Ferguson lot. I showed him a security clip of the white truck plowing in the parked vehicle and Negoshian stated that he doesn't have any white trucks that plow for him only black ones. I then traveled to the City of Newton Highway Department and spoke with Superintendent Michael Arpino. I showed him the security video and he stated that the white truck in question was not a city owned or sub-contracted vehicle because the trucks would have a green flag on their mirrors or writing on the door. I also spoke with Newton Control Clerk Mark Parsons who viewed the City of newton GPS from the night of the storm and confirmed that no city vehicles or sub-contracted vehicles were on Ramsdell St between 2am and 3am. Unless new information on the identity of the white truck is learned, the vehicle in question (veh#2) will remain unknown. On 12/28/20 I spoke with Elias Ghazale's wife, Maria Vidalis wife who stated she was able to uncover the snowed in vehicle and found the following damage to Massachusetts Reg # 1WPG55: drivers door, front bumper, rear quarter panel scratches, drivers side fender and reports of leaking transmission fluid.. I then traveled to #964 Boylston St and viewed the vehicle myself and all confirmed the above damage except the transmission fluid.

The traffic signal was cracked at it's base, and still functioning while resting on the sidewalk. The traffic signal when operation faces traffic traveling Westbound on the Commonwealth Avenue carriage road at Auburn Street. An anonymous resident of the area approached me and showed me a picture on their phone of the truck that left the area after crashing into the traffic signal. The photo was of a large City of Newton orange dump truck with a plow affixed to the front of it. I could not make out the registration of the vehicle, but I could see the number 48 on it's side to identify it. I requested Newton Dispatch contact the DPW to identify who the operator of the vehicle was. A message was left for Mr. Perry Cacciola (Newton DPW) for information about the crash. On Monday, January 10, 2021, I spoke with Mr. Cacciola. Mr. Cacciola stated Donald Leblanc (S00405419) was the operator of the plow responsible for the damage to the traffic signal. Mr. Cacciola stated he could not provide any information about the vehicle used in the crash, except that it was number 48. I then spoke with Mr. Leblanc. Mr. Leblanc stated he was operating the City of Newton plow involved in the crash. Mr. Leblanc stated the snow at the time of the crash was packed down and the plow made contact with the traffic signal while he was clearing the snow in front of 1897 Commonwealth Avenue. Mr. Leblanc stated he contacted his supervisor, Perry Cacciola, sometime after the the incident to reprot the damage. Mr. Leblanc reported no injuries. Photos were taken of the damaged traffic signal and submitted to the IT Bureau.

Narrative:
On 02/01/22 at 14:45 hours, I responded to 1025 Walnut St for a reported city motor vehicle crash involving a student transportation van and another vehicle. The weather at the time was 32 degrees and cloudy. The road surface was wet from a recent heavy snowfall. On arrival, I spoke to the operator of Vehicle # 1 identified as Gary Moresco (MA D/L # S26008460). He was working for JSC Transportation Services Inc. who provides transportation for City of Newton students. At the time of the crash he was transporting a student (Christopher Manthei) from Dearborn Academy back to his residence. He was operating a 2016 Dodge Grand Caravan color white bearing MA Pupils reg. # 104232. I asked him for his version of events and he stated that traffic was moderate at the time and he was heading westbound on Walnut Street. Next, he says that he stopped due to Vehicle # 2 stopping in front of him. Vehicle # 2 then started moving and suddenly came to a stop again. He was then not able to stop in time. He then struck the rear end of Vehicle # 2 causing front end damage to the entire length of the car which included the bumper, grille, and hood area. Mr. Moresco reported no injuries along with the student (Manthei). Manthei was checked out by Fallon Ambulance personnel and his mother signed a patient refusal on his behalf. The mother, Jennifer Manthei, then took her son home. Operator of Vehicle # 2 was identified as Lynn Slobodin (MA D/L # S98417005). She was operating a gray 2015 Toyota Rav4 bearing MA reg. # 3EB379. A passenger with her who was sitting in the front passenger seat was identified as Julia Hass. I asked Ms. Slodobin what had occurred and she stated that she was traveling westbound on Walnut Street and a vehicle in front of her suddenly stopped. She then stopped her vehicle and that is when she was rear ended by the Dodge. Her Toyota sustained rear end damage to the trunk and entire bumper area. Ms. Slobodin complained of head and neck pain along with pain on the top of her right thigh. Ms. Hass complained of head and neck pain also. Both were checked out by Fallon Ambulance personnel and they signed patient refusals. While investigating the crash, Ofc. Colella stated to me that a witness approached him identified as Brenda Noel. Ms. Noel resides at the crash location. She stated to Ofc. Colella that once she heard the crash she came out and saw the operator of the Dodge Grand Caravan on his phone. She says that he was holding the phone up to his head with his right hand. I then asked Mr. Moresco (Dodge operator) if he was on his phone prior to the crash and he replied that he was not. He then showed me his call log after asking for it and it showed only one entry at 14:53 hours. He states he was reporting the accident to his employer at that time. There were no other entries in his call log. Next, I looked at the front of Mrs. Noel's residence and observed a camera to the right of her doorway. I then knocked on her door and inquired about possible footage of the crash and she stated only her husband knows how to work the system. I then asked her again what she had seen. She states that once she heard the crash take place she saw the operator of the Dodge holding up his right hand to his head. She stated that she never saw an actual phone. I then gave her my information to call me regarding possible footage. She then called back later and stated that the camera does not show anything. Photos taken of the scene and submitted to the I.T. On Wednesday 2/2/22 at approximately 1137hrs while assigned to marked unit n498 I was driving Eastbound on Nahanton Street when I was flagged down by a motorist operating a Black BMW SUV. Operator of the vehicle, identified as NOVAIS Eduardo, states he was driving Westbound on Nahanton Street following a Smart Car when "out of nowhere" the Smart Car made a U-Turn in the middle of the road reversing direction on Nahanton Street. NOVAIS states the only way he could avoid a collision with the car was pull hard to the right which he did, striking a snow bank on the side of the road. NOVAIS states there was a verbal exchange between he and the female driver, but she drove away. NOVAIS took a picture of the vehicle's license plate, which is EV5791. As of the time of this report I have been unable to speak with the operator of that vehicle. NOVAIS is concerned about damage to his vehicle. Although there was no physical body damage to his vehicle there was a message displayed on his dashboard stating there could be issues with the Chassis and he should schedule maintenance right away. NOVAIS further states he could feel something unusual with the steering, but felt the vehicle was safe to drive as he lived close to Nahanton Street.

Upon arrival the operator of MV1 stated that he was driving on the right lane of Washington St and turned right to merge onto Center St (S/B). The operator of MV1 stated that the operator of MV2 was in the middle lane when he tried to merge onto the right lane which caused MV1 to hit MV2 on the right side of the bumbler. As they continued driving straight MV2 stopped which caused MV1 to tap the rear bumper of MV2 Causing minor damages to the bumper. The operator of MV2 stated that he was the one driving on right lane when the operator of MV1 hit him in the rear bumper. The operator of MV2 stated that he had the right away which is why MV1 crashed into his bumper. I asked the operator of the of MV2 how did he get damage on the right side of his vehicle to which he replied he was not sure, and that it was there already. He then continued to say that he must of hit the snow bank which caused the damage but the snow bank was a lot lower than where the damage was. Based on my observation the damage is consistent with what the operator of MV1 stated. Medics arrived on scene and both operators denied medical attention.

The owner of MV1 was inside the Shaking Crab located at 203 Adams street when he came outside and observed damage to the drivers side of his parked vehicle. A good Samaritan witnessed the accident and stated to him, a SUV struck the reporting parties vehicle and took off. The witness took video of the accident. I watched the video and observed the SUV hit MV1 with the passenger side of the vehicle and then brake and continued southbound on Adams st. The witness got a registration of MA reg 2BZH27 and told the owner of MV1 that a female was operating the vehicle. Dispatch attempted to contact the owner of the vehicle yielding negative results and unable to leave a voicemail. A text message was sent to the owner to contact NPD from the dispatch cell phone. MV1 had damage to the driver side doors and mirror. A voicemail was left for the witness. I forwarded a video of the accident and photo of the vehicle to IT to be added to the report. As of now the owner of MV2 is listed as "uninvolved owner" under persons "2" until the accident is further investigated and until contact is made with the owner to determine who was operating the vehicle. Charges pending. Traffic Bureau update (Officer Gaudet): On March 11, 2022, I reviewed the dash camera footage submitted to the IT Bureau by Officer Stake. The footage shows a dark colored SUV vear off to the right side of the roadway on Adams Street. The vehicle's front passenger side crashes into the driver side of a parked vehicle in front of 203 Adams Street (S). As the witness drives by where the crash occured, the footage from their camera pans over to the parked vehicle and shows damage to it's driver side. The footage then resumes facing Southbound on Adams Street and the fleeing vehicle continues Southbound on Adams Street towards Washginton Street and does not stop. The vehicle is idenitified by a still photo submitted by the witness as a 2016 Kia Sportage bearing Massachusetts registration 2BZH27. This vehicle is registed to Ms. Jennifer Mc Quiggan (S30952065). I contacted Ms. Mc Quiggan and asked about her vehicles involvement in the crash on March 10, 2022. Ms. Mc Quiggan stated she was operating the vehicle at the time of the crash on Adams Street and thought she hit a snow bank. I advised Ms. Mc Quiggan that I had just finsihed reviewing the footage of the crash that was submitted by a witness and it shows her vehicle vearing off of the road and crashing into a parked vehicle. After reviewing the footage of the crash and obtaining an admission by Ms. Mc Quiggan that she was operating MV2 at the time of the crash, I issued Ms. Mc Quiggan Massachusetts Uniform Citation 328660AB for Chapter 90, Section 24 (Leaving the Scene of Property Damage) and Chapter 89, Section 4A (Marked Lanes Violation). Adams Street is a public way maintained by the City of Newton.
