On Sunday, February 3rd 2019, at approximately 11:45pm, I, Officer Brooks, responded to a single car MVA at the intersection of River street and Auburndale avenue. Upon arrival I observed MV1(MA REG 7TT558) in the travel lane with both tires broken free from the axle. I spoke with the operator who could not recall how the accident happened, or what was struck. After an investigation the operator was placed under arrest for 90/24 OUI Alcohol. Officer Boyle was able to assist in the accident investigation. It appears the operator had been traveling westbound on River street and struck a utility pole on the drivers side front tire. The vehicle continued to the intersection of River street and Auburndale avenue and struck a curb here on the passenger side tire. Both tires were broken free from the axles. The vehicle was towed by Tody's and an inventory form was filed. Eversource was notified about the pole.

On Sunday 3/17/19 at approximately 1606 while assigned to marked unit n499 I was dispatched to the area of 89 Wiswall Rd. for a report of a two car motor vehicle crash with no injuries. Upon arrival I spoke with WONG Cheung, the operator of motor vehicle #1. WONG states that he was traveling southbound on Wiswall Rd. from Dedham Street when he was attempting to pull into his driveway at 89 Wiswall Rd. WONG further states that as he attempted to turn Motor vehicle #2 passed him on the inside causing him to strike motor vehicle#2 on the drivers side. WONG stated to me that he used his right turning signal. Operator of motor vehicle#2, CAI Wei, states that he was traveling behind motor vehicle#1 and he thought that motor vehicle #1 was taking a left into 88 Wiswall Rd. He states that Motor vehicle #1 made movement to the left toward the center yellow double line as if he was turning left so he attempted to pass motor vehicle#1 when it suddenly turned right into 89 Wiswall Rd. causing him to strike the front passenger side of the motor vehicle. CAI states that motor vehicle #1 never used a turn signal.

On Thursday, April 11, 2019 while assigned to Traffic unit N525, I responded to the area of 1302 Washington Street, Newton for a report of a motor vehicle accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Washington Street is a public way maintained by the City of Newton. The operator of MV1, Jacqueline Dupre (S29256883), stated she was parked in front of 1302 Washington Street (E) in her 2018 Honda CRV (MA: 5054RF). Dupre stated she was attempting to exit her street parking spot and did not see any vehicles traveling towards her as she entered Washington Street. Dupre stated MV2's passenger side hit her front driver side area as she was attempting to enter the lane. I observed minor damage to the front driver side quarter panel/headlight area. Dupre reported no injuries at this time. The operator of MV2, Donnell Stoute (S53698210), stated he was operating a 2017 Porsche Macan (MA: 7ZC447) East on Washington Street. Stoute stated as he was traveling by 1302 Washington Street, MV1 pulled out of her parking spot and hit the passenger side of the vehicle he was operating. I observed moderate damage to the passenger side of MV2. Stoute reported no injuries at this time.

I responded to the area 107 Dartmouth St. at approx. 0345 on 04/28/9019 along with N493 Officer Kelly Donahue and N508 Sgt. Devine for a report of an MVA. The MVA was reported to be single vehicle crash. The vehicle left the roadway and was on the front lawn of 107 Dartmouth St. I arrived on location (first on scene ). I observed tire tread marks across the front lawn of 107 Dartmouth St. I then observed a vehicle in the bushes toward the left front side of the house. The vehicle; NY Reg. JHE8252, Porsche color blue. The RMV info on the plate came back registered to Porsche;of ;South. Registration plate type: Dealer Assignment. 185 W Sunrise HWY; Freeport,NY (no further info). The vehicle was unoccupied and secured. I felt the hood of the car which was still warm from the engine. I checked the immediate area surrounding the crash scene for an operator but found no one. I radioed to other responding units to canvas the neighborhood and surrounding area for the operator on foot. I spoke to the homeowners at 107 Dartmouth St., identified as Anthony and June Limberis. Mrs. Limberis stated she heard a load noise and a car engine revving. She stated when she looked out the window she observed a W/M tall, thin, wearing tan khakis and a light colored plaid shirt walking across the front lawn. He proceeded up Dartmouth St. (northbound) toward Commonwealth Ave. Newton Fire Dept arrived on location to check the crash scene and vehicle for possible leaking fluids and any other hazards. As I was talking with the firefighters we were approached by two men. One of the men was intoxicated. He was unsteady on his feet, had glassy bloodshot eyes, and was slurring his speech. The man was a W/M tall/thin approx. 6'2" and was wearing tan khakis and a light colored plaid shirt. He was identified as Eric Adams DOB 09/18/1969. The second man with him was identified as John Adams DOB 02/07/1936; An elderly W/M approx. 5'6" also wearing tan khakis and a light colored plaid shirt. Eric Adams stated his father John was the operator of the vehicle. Eric Adams had a strong odor of Alcoholic beverage emanating from his person and breath. He continued to slur his speech and was unsteady on his feet as he spoke. He was swaying back and forth and side to side as he spoke to me. Officer K. Donahue and Sgt. Devine were on scene at this time and also made the same observations. Multiple times we had to grab hold of Eric Adams to keep him from losing balance and falling. We spoke with Eric and John Adams for several minutes as their accounts of what took place were not consistent. I asked Eric Adams where the keys to the vehicle were. Eric Adams immediately pulled the starter key to the vehicle from his front right pocket. Eric Adams became defensive and belligerent when questioned about who was operating the vehicle. John Adams appeared tired (as if he just woke up) and confused as to the situation. Sgt Devine and I both spoke with Eric Adams and John Adams. Although both men claim John Adams was the operator of the vehicle, a clear determination was uncertain. A query of both mens driver's license status on NY Licenses came back active. Eric Adams has two prior DUI's on his RMV record. Officer Donahue and Sgt. Devine escorted the two men to 93 Dartmouth St. Eric Adams stated they were staying at that residence which is his sister's house. I stayed at the crash scene to document information and assist with clearing the MV from the property with Tody's Towing Service. As they were walking away I observed Officer Donahue and Sgt. Devine continue to have to assist Eric Adams with staying on his feet. Approximately ten minutes later as I was speaking with the aforementioned homeowners Mr.&Mrs. Limberis. Officer Donahue and Sgt. Devine returned to the crash scene. As they were updating me we observed Eric Adams returning to the crash scene. He was stumbling and swaying as he walked and repeatedly almost fell. Officer Donahue and Sgt. Devine immediately intercepted and assisted him before he fell. As they were assisting him again I continued speaking with the homeowners. Suddenly I heard Officer Donahue and Sgt. Devine struggling with Eric Adams who was now on the ground and resisting being handcuffed. I ran to assist them. Eric Adams continued to resist and be uncooperative until he was appropriately restrained in handcuffs. Eric Adams was placed under arrest by officer Donahue for Disorderly Conduct and Resisting Arrest. Reference Officer Donahues Newton Police Incident Report #19016484. The vehicle NY Reg. JHE8252 was removed from the property of 107 Dartmouth St. and towed from the crash scene by Tody's Towing Service.

MV1 was traveling east on Madison Ave from Walnut St approaching #80. There were 2 landscape vehicles parked on left side of the street. The operator, Sarah Ruth Loper-Sengupta, stated that she never saw the pedestrian that was in the roadway (N) due to solar glare. Loper-Sengupta stated she heard a thump coming from the passenger side of her vehicle. Loper-Sengupta stated she looked over to her right and saw a trash barrel in the air. Loper-Sengupta stated she then saw a male party lying on the roadway on her passenger side. Immediately after striking the pedestrian, Loper-Sengupta, stopped to check on him. There was a golden doodle dog in the back seat of the MV at the time of the accident, unrestrained. There were no obstructions hanging from the mirror or windshield area. The front drivers visor was down. There was no airbag deployment. I observed significat damage to the front passenger side windshield area. There was also damage to the front passenger side hood area. See Sgt. Lee update for victim statement Numerous pictures were taken by Ofc. Gaudet of the MV and scene. They were TOT IT. The MV was towed by Tody's due to its shattered windshield. No citation will be issued to Loper-Sengupta. The solar glare caused her to not able to see the pedestrian. The Sgt. and I dont believe the pedestrian looked before crossing with the barell on his left shoulder because he would have been able to see the MV coming as this is straight away section of Madison Ave. It should be noted, that when I arrived on scene I immediately noticed the seolar glare as I walked east on Madison Ave.

On Tuesday May 7th, 2019 at approximately 13:15 hours I responded to the area of 30 Langley Road for an auto vs. pedestrian accident. At the time of the accident the weather was sunny and the road conditions were dry. Langley Road is a public way maintained by the City of Newton. Upon arrival I first spoke to the pedestrian who was inside the ambulance getting medical attention. The pedestrian identified as Donna Kalikow (S24151638) stated she was standing in an open parking space for a few seconds, attempting to walk across the street to enter J.P. Licks. Donna stated she was standing in the marked parking space waiting for traffic to stop to safely cross the street. Donna states the cars stopped to let her cross and she proceeded into the roadway. She states MV1 was attempting to back into the parking space that she was standing in and struck her right side and knocked her to the ground. Donna states the operator of MV1 stopped the vehicle when he realized he had struck her and got out to contact 911. Donna appeared to have minor injuries and complained of head pain. Medics transported Donna to NWH to be further evaluated. The operator of MV1 identified as Christopher Moos (S24090966) stated he was attempting to parallel park into an empty parking space and never saw the pedestrian standing in the space. Christopher stated as he was backing up into the space he was notified by a bystander that he just struck a pedestrian. Christopher stated he immediately stopped the vehicle and got out to assist the pedestrian and contact 911. Christopher stated he was not travelling no more than 5mph at the time of the accident. MV1 did not have any damage to the vehicle and was able to leave the scene. Christopher reported no injuries. Several photographs were taken of the scene and submitted to the IT Bureau. I mailed the operator a Massachussetts Uniform Citation for a City of Newton Ordinance violation for Section 19-75 Failure to use care when backing.

On 05/10/19 I met with Luiz A. Lopes who stated his vehicle ( Mass Commercial Reg# T83695 White Toyota Tacoma P/ U) was struck while parked in the lot around 07:00. Lopes said he didn't call the Police right away because he wasn't sure his car was struck there. The truck sustained a cracked front bumper and the fog light was popped out. We went inside of National Lumber and spoke with a manager who said he would contact G.M. Greg Assarian and have him look at the security footage. On 05/14/19 at approximately 14:00 the Police Station received a call from Lopes who just got off the phone with Assarian who found some security footage of the incident. I then traveled over to National Lumber and met with Assarian. He showed me the video and it appears that a newer grey four door Chevrolet truck backed into Lopes truck around 06:58 of 05/07/19. The Massachusetts plate is unreadable but an older White heavy set male gets out of the truck and walks into the store. After parking the truck the male can be seen checking his own truck bumper for damage. The male is wearing a baseball hat and sun glasses and cannot be identified on the video. The male does however walk with a noticeable limp. Assarian will show his picture to his employees and contact me if the employees are able to identify the male driver. I called Lopes back and apprised him of my findings. ******************UPDATE**************************************** As of today 05/20/19 I have yet to hear back from anyone at National Lumber so I will end this investigation and label it a hit and run crash.

Tipruthai Thangthong stated that on 05/21/2019 at approximately 16:36 hours she was pulling into the driveway of her sisters home at 53 Rangeley Rd while operating her vehicle MA reg 2LVL61 a 2018 Honda Accord. Thangthong stated that her three old son Mighty Mettanant was a passenger in her vehicle. Thangthong stated that she was driving approximately 5 MPH as she drove into the driveway. The driveway runs east to west and Thangthong's vehicle was heading west into the driveway as the sun was in the Western sky creating a glare. Thangthong stated that she did no see anything in the driveway prior to her pulling into it. Thangthong stated that she felt the front of her vehicle strike what she described as " something " and stopped her vehicle. Thangthong stated that she put her vehicle in reverse and then into park and exited her vehicle to see what she had struck. Thangthong stated that she saw her two nieces, Kalla Mongkhonlertsirkul age three and Kelley Mongkhonlertsirkul age seven laying in the driveway in front of her vehicle. Thangthong stated that she then realized that her vehicle had stuck her two nieces as they were in the driveway. Both of the Mongkhonlertsirkul sisters were conscious and alert and were able to walk to an ambulance for transport to Newton Wellesley Hospital for treatment. Thangthong stated that she and her son were not injured in the crash. I observed no damage to Thangthong's vehicle. I responded to the Newton Wellesley Hospital and spoke with the Mongkhonlertsirkul sisters and their mother Piyada Kellum. Kellum stated that her oldest daughter Kelly is a student at the Franklin Elementary School . Kellum stated that her daughters told her that they were playing in their driveway when their aunt's vehicle entered the driveway and did not stop. I spoke with both of the Mongkhonlertsirkul sisters who told me that they tried to get out of the way of their aunt's vehicle but could not in time. The Mongkhonlertsirkul sisters stated that they were both struck in the back and fell to the ground between the two front tires of their aunt's vehicle. Kalla Mongkhonlertsirkul had scrapes to her right knuckles. Kelly Mongkhonlertsirkul had lacerations to her lower left arm and the left side of her face near her eye. I spoke with hospital staff who stated that there did not appear to be any broken bones or other internal injures. I took photos of the crash area and forwarded the disk to the NPD's IT bureau for downloading.

ON MY ARRIVAL TO THE SCENE I SPOKE WITH THE OWNER OF VEHICLE #1 IDENTIFIED AS ROBERT RANKINGS. HE REPORTS THAT WHILE EATING LUNCH IN TOMMY DOYLES A WOMEN(WITNESS), LATER IDENTIFIED AS PATTY TUCKER ENTERED THE RESTAURANT ASKING IF ANYONE OWNED A RED TOYOTA PARKED OUT FRONT? HE REPLIED, "HE DID". WITNESS THEN TOLD HIM SHE WITNESSED ANOTHER VEHICLE HIT HIS CAR THEN DRIVE AWAY. MS TUCKER(WI) TOLD MR RANKINGS THAT WHILE TRAVELLING W/B ON WATERTOWN ST SHE WITNESSED SUSPECT VEHICLE WHO WAS GOING IN THE OPOSITE DIRECTION(E/B) ATTEMPT TO MAKE A U TURN IN FRONT OF HER. SUSPECT MADE TWO ATTEMPTS MAKING HIS U/TURN THE SECOND ATTEMPT HE CRASHED INTO HIS VEHICLE. WITNESS TOOK A PICTURE OF TH SUSPECTS LICENSE PLATE, MA REG 31Y660 AND GAVE IT TO MR RANKINGS. A REGISTRATION INQUERY BY POLICE SHOWED THE VEHICLES REGISTERED OWNER AS OUCIBLE OUSSAMA, DOB 12/16/1984, ADDRESS OF 62 DEHON ST, REVERE MA, 02151.A BOLO WAS PUT OUT ON THIS VEHICLE WITH NO SUCCESS. REVERE PD WAS NOTIFIED TO CHECK SUSPECT VEHICLES ADDRESS WITH NEGATIVE RESULTS. THERE WAS NO TELEPHONE LISTING FOR THE REVERE ADDRESS. THE FOLLOWING DAY,JUNE 7TH, 2019, AT 09:30HOURS I SPOKE WITH THE WITNESS, PATTY TUCKER. MS TUCKER GAVE THE FOLLOWING STATEMENT TO ME REGARDING THIS INCIDENT: I WAS GOING W/B ON WATERTOWN ST HEADING TOWARDS WEST ST WHEN SUSPECT VEHICLE WHO WAS COMING E/B ON WATERTOWN ST(OPOSITE DIRECTION) SUDDENLY MAKE A U TURN IN FRONT OF HER. SUSPECT VEHICLE HAD TO MAKE TWO TURNS TO COMPLETE HIS U/TURN HOWEVER ON THE SECOND ATTEMPT HE STRUCK THE VICTIMS PARKED VEHICLE. SUSPECT THEN DROVE A SHORT DISTANCE AWAY STOPPING IN FRONT OF MAGNI FUNERAL PARLOR. SUSPECT, WHO WAS DESCRIBED AS A W/M, IN HIS THIRTIES WITH A SHAVED HEAD AND WEARING SUNGLASSES GOT HALFWAY OUT OF HIS VEHICLE, LOOKED BACK THEN GOT BACK INTO HIS VEHICLE AND DROVE AWAY. MS TUCKER WAS ABLE TO TAKE A PICTURE OF THE SUSPECTS LICENSE PLATE WITH HER CELL PHONE PRIOR TO HIM FLEEING. SHE LATER PROVIDED IT TO THE VICTIM, ROBERT RANKINGS. I WILL BE MAILING A CITATION#T1444062 TO THE REGISTERED OWNER FOR LEAVING THE SCENE OF ACCIDENT, 90/24C AND MAKING AN ILLEGAL U/TURN, CITY ORDINANCE VIOLATION 19/86

Vehicle #1 stated they were attempting to turn around on beacon St. and turned into the driveway at 865 Beacon St. Vehicle #1 stated they were reversing out into the road and was stopped and shifting into drive when they were struck from the rear by Vehicle #2. Vehicle #2 stated she was traveling westbound on Beacon St. when she lost sight in front of her due to the glare from the sun. Vehicle #2 stated she never saw Vehicle #1 until she struck the vehicle. Vehicle #1 had minor damage to the rear and was able to be driven away. Vehicle #2 had moderate damage to the front and was towed by Tody's Towing. Operator of Vehicle #1 stated she was not hurt in the crash and refused any medical treatment. Operator of Vehicle #2 stated she had chest pain and was evaluated by Medic 1. Operator of Vehicle #2 was cleared by the medics and refused any further treatment.

On Monday, July 1, 2019 while assigned to Traffic unit N525, I responded to the area of 802 Walnut Street, Newton for a report of a motor vehicle accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Walnut Street is a public way maintained by the City of Newton. The operator of MV1, Dayana Martinez (S96710554), stated she was operating her 2019 Toyota Rav 4 (MA: 934RJ9), Northbound in the area of 802 Walnut Street, Newton. Martinez stated as she was coming around the bend the sensor system for her vehicle activated and she attempted to pull her vehicle away from the double yellow line. Martinez stated the vehicle locked up and she made contact with MV2. Martinez reported no injuries on scene. I observed moderate damage to the front driver side area of MV1. The operator of MV2, Gerard Kenneally (S75917793), stated he operating his 1999 Ford E150 (MA: 3AL821), Southbound in the area of 802 Walnut Street, Newton. Kenneally stated MV2 was traveling Northbound on Walnut Street and crossed over the double yellow line and side swiped the driver side of his vehicle. Kenneally reported no injuries on scene. I observed moderate damage to the driver side of MV2 from the driver door to the rear bumper area. I spoke with a witness to the accident, Bryan Stewart, via telephone. Stewart stated 3/4 of the vehicle was in the Southbound lane while it was traveling Northbound. Stewart stated MV1 crashed into MV2. Stewart stated if MV2 was not in front of him then MV1 would have crashed into him. After speaking with all parties involved, Martinez was mailed Massachusetts Uniform Citation T1272998 for Chapter 89, Section 4A (Marked Lanes).

On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

Mullings stated the rear passenger side bumper area of MV1 made contact with a unoccupied motor vehicle parked Eastbound on Beacon Street in a metered spot across from 837 Beacon Street. No injuries were reported by the occupants of Ladder 2, I observed minor damage to the rear passenger bumper area of MV1. I spoke with the owner of MV2, Pakizeh Motiee (2017 BMW X3, MA: 7NCC90). I observed minor damage to the front driver side quarter panel area of MV2. Motiee was advised. Pictures of both vehicles were taken and submitted to the IT Bureau.

Orama stated the vehicle behind him attempted to pass him on his right and the front passenger side of MV2 crashed into the rear driver side of his vehicle. I observed minor damage to the rear driver side bumper area. Orama reported no injuries. The operator of MV2, Michael Ansel (S11557140), stated he was operating his 20003 Mercury Marquis ( MA: 992SK2) near 2000 Commonwealth Avenue (W). Ansel stated he was having an emergency and was trying to get around traffic to use a restroom at a local business. MV2's front passenger bumper crashed into MV2's rear driver bumper area. Ansel reported no injuries. Ansel stated he was responsible for the accident to me.

On 09/27/19 at 08:41 hours while assigned to Traffic Unit 523, I responded to a motor vehicle/bicyclist accident at 1330 Centre Street. The weather at the time was clear and sunny. The road surface was dry and Centre Street is a public way maintained by the City of Newton. Traffic conditions were at a heavy volume and Centre Street was congested at the time. There is no dedicated bicycle lane in this area due to the road being too narrow. I spoke to the bicyclist involved (Unit 2), Michael Hnatko (S10432772). Mr. Hnatko stated he was traveling northbound on Centre Street on the right side of the road on his 20 inch gray Trek bicycle. He further says that he was peddling very fast at the time and must have not been paying attention to MV1 turning right in to parking lot of 1330 Centre Street as he was approaching from behind. As MV1 was turning right, he then struck the passenger side rear quarter panel of the vehicle and fell forward off of his bicycle. He sustained minor injuries such as scrapes to his knuckles on both hands and a small cut to the bridge of his nose. He signed a patient refusal with Cataldo Ambulance. It appeared that the front alignment was now off on his Trek bicycle. I then spoke to the operator of MV1, Lonnie McGinnis (S66044083). He states that he was inching along in heavy traffic waiting to turn right in to the lot of 1330 Centre Street with his right turn directional signal on. As he was turning right, the bicyclist struck his vehicle on his passenger side rear quarter panel area. McGinnis thought it was clear to turn right at the time and never saw the bicyclist approaching from behind. No injuries reported by McGinnis and I observed minor dents and scratches to the passenger rear quarter panel area. Digital photos taken by Ofc. Gaudet.

On Wednesday October 23rd, 2019 at approximately 1306 hours I responded to 40 Beaconwood Road for a motor vehicle accident involving a city vehicle. At the time of the accident the weather conditions were sunny and the road conditions were dry. Beaconwood Road is a public way owned by the City of Newton. Upon arrival I spoke with the operator of the city vehicle (MV1) identified as Norayr Sargsyan ( S34138376). Norayr stated that he was traveling southbound on Beaconwood Rd operating a yellow 2009 Ford drwsup truck (MA COM REG; M2939A) and as he passed a landscaping truck that was parked, the rear drivers side corner of the city truck made contact with the rear drivers side of the landscaping trailer. Norayr stated there was a car parked facing the wrong direction directly across from the trailer making it hard for him to pass in between both vehicles. No damage to the city truck was visible, minor damage was present on the trailer. Photographs were taken of the damage and submitted to the IT bureau. No injuries were reported. I spoke with the operator of the landscaping truck (MV2) identified as Jose Nunez (S24348117). Jose stated MV2, a red 2008 Ford drwsup dump truck was unoccupied and parked with a red 2012 Brav utility trailer (MV3) attached to it. Jose reiterated the same sequence of events as Norayr. Both vehicles were able to leave the scene without further incident and I advised both parties a report would be submitted.

Moore stated she did not see MV2 passing by as she was backing out and the rear driver side bumper area of her vehicle crashed into the rear driver side of MV2. Moore reported no injuries. I observed moderate damage to the rear driver side bumper area of MV1. The operator of MV2, Patrice Lys, stated he was traveling Westbound in the parking lot of the Angier Elementary school in a 2018 Ford EcoVan (MA MVN: M2546A). Lys stated a vehicle on his driver side backed out of a parking spot and the rear end of MV1 crashed into the rear driver side of MV2. Lys reported no injuries. I observed minor damage to the rear driver side area of MV2. The operator of MV1 was cited with Massachusetts Uniform Citation T2016428 for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care, Backing). Photos were submitted to the IT Bureau.

Upon arrival I spoke with the operator of motor vehicle 2, GIRARD Kathleen, who states that she was driving from the 2nd aisle of parking spots into the main travel lane to attempt to exit the mall. When she attempted to turn left she didnt see motor vehicle 1 already in the lane of traffic striking the side of the vehicle. After speaking with GIRARD I spoke with the operator of motor vehicle 1, SONG Guang min. SONG did not speak english, but he corroborated what GIRARD had stated by using a translation app on his cell phone. GIRARD had damage to the front of her vehicle and SONG had damage to the drivers side of his vehicle. Both vehicles were still operable and were driven from the scene. Neither party was injured

On 11/26/19 at 14:11 hours, I responded to Newton South High School located at 140 Brandeis Road for a two car motor vehicle accident involving a school transport van for the City of Newton and another vehicle. No injuries were reported. The road conditions at the time were dry and the weather was a mix of sun and clouds. Traffic conditions were heavy due to the release of school at the time. Operator of MV# 1 states he was operating straight ahead eastbound on Brandeis Road awaiting the backed up traffic in front of him that was entering the right roundabout for the main entrance to the high school. While doing so, he states that MV # 2 came up and passed him on the left to go around the traffic. MV # 2 then clipped his vehicle on the driver's side front quarter panel while passing him. I observed no visible damage to the quarter panel area and scuff marks to the left front rim which possibly were from the accident. Operator of MV # 2 states she was also operating straight ahead eastbound on Brandeis Road awaiting the backed up traffic in front. She states that once there was an opening in the roadway ahead of her, she signaled her left directional and started to pass traffic on the left. While doing so, she says that MV # 1 suddenly pulled out to the left without signaling and struck her vehicle on the front passenger side. I observed minor scuff marks to the front passenger quarter panel area and slight damage to the metal grate footstep on the passenger side. The operator was also transporting two students for the City of Newton through JSC Transportation Services at the time. Both students attend Newton South High School. Operator of vehicle one, Christopher Sermak, stated that while turning north bound in the parking lot of 175 Boylston St he was blinded by solar glare. Sermak stated that the diver's side visor was down and the he was not wearing sun glasses. Sermak stated that he heard a loud bang coming from the front driver's side of his vehicle and realized that his vehicle had stuck something. Sermak stated that he stopped his vehicle and saw a male identified as John Baker laying on his back on the ground. Baker stated that he had step off the side walk in front of 175 Boylston St and was walking to his car when he saw Sermak's vehicle approaching him and realized it was not slowing, Baker sated that the front driver's side of Sermak's vehicle struck his right arm and shoulder causing him to be knocked to the ground on his back. Baker stated that his head struck the ground but he did not lose consciousness. Baker was transported to Newton Wellesley Hospital via EMS for treatment. While on scene I observed that the rear passenger side brake light of Sermak's vehicle was out. I issued Sermak MA Citation T 2012527 and cited him for violations of City of Newton City Ordnance 19-75, failure to use care while tuning , and MGL 90/7 , defective lights.

At approximately 1645HRs on Tuesday, 01/07/2020, I was dispatched to 325 Boylston St. (Sunoco Gas) for a report of a hit and run motor vehicle crash. Upon arrival, I spoke with the operator of MV1 (MA Veteran:J706), who stated that as he sat stationary in his vehicle just before entering pump: 3, MV2 backed up from pump: 1 and struck MV1 in the front bumper, despite him activating his horn and keeping it active as she continued to back up. The operator of MV1 then approached the driver's side door of MV2 to exchange information, however the operator of MV2 drove off; the operator of MV1 provided MA Pass: 8SV842 being attached to MV2 and I noted damage to the front bumper of MV1. I spoke to witness 1 via phone and he concurred with the operator of MV1's account of the incident. It should be noted that witness 1 could see the crash well from his vantage point by pump: 2. The operator of MV2 could not be reached by this officer for comment. Natick PD responded to the operator of MV2's residence and observed damage to MV2's rear bumper. They requested the operator of MV2 contact me via phone and she stated she would; Natick PD did not get a contact number for her. Natick PD informed us to expect her call and a few minutes later, dispatcher Kelly stated that she received a call from the operator of MV2, who immediately hung up once the call was answered. It should be noted that since she called on the business line and immediately hung up, we were not able to retrieve her phone number as the system does not store it. Due to the operator of MV2 failing to provide her information after the crash/fleeing the scene and her not being cooperative with my investigation, I have issued her by mail MA Criminal Citation:T2013367 for violating one count of MGL. Ch. 90 S. 24 (Leaving The Scene Of An Accident After Property Damage). Note: the clerk at Sunoco stated that the cameras at the pumps do not store footage. **Update** At approximately 1540HRs on Thursday, 01/07/2020, I spoke with the operator of MV2 via telephone. She contacted the Newton Police Traffic Bureau and provided her phone number, who in turn forwarded it to me. She stated to me that on the night in question, she was at the pumps and the pump she initially wished to use (pump: 1) had a sign on it to pay inside. She then elected not to use said pump and backed her car up and heard the operator of MV1 activate his horn. She stopped and was unaware that she struck MV1. When she saw the operator of MV 1 exit his vehicle, she assumed he was going to pump gas. She then stated she left the gas station because she was frustrated and did not intend to leave the scene of an accident. I advised the operator of MV2 that since I was unable to speak with her that night, I had already issued her MA Criminal Citation:T2013367 for violating one count of MGL. Wint stated that as he approached the area of 277 Lexington St he believed that the traffic light that is at the marked crosswalk was green for south and north bound traffic. Wint stated that he did not see any pedestrians in the crosswalk, until he heard the sound of an object impacting into the front driver's side of his vehicle. Wint stated that he stopped his vehicle approximately ten yards past the crosswalk and got out. Wint stated it was then he realized that his vehicle had stuck a female and a dog. Wint stated that he was not operating any electronic devices at the time of the crash, and I observed none in his vehicle. Wint stated he was not injured in crash. Wint's vehicle had damage to the driver's side mirror and lower molding of the driver's side front door. Wint's vehicle did not require a tow. Witness Bessy Shanks stated that she was driving in her vehicle behind Wint's vehicle south bound on Lexington St. Shanks stated that Wint's vehicle was driving slow and swerving approximately two blocks prior to the crash. Shanks stated that she believed that Wint may of been distracted. Shanks stated that south and north bound traffic was stopped because that cross walk light was red and that a female and a dog were crossing from the east to west in the marked crosswalk in font 227 of Lexington St. Shanks stated that the front driver's side of Wint's vehicle srtuck the female and the dog as they were approximately halfway into the crosswalk. Shanks stated that the female and dog both fell to the ground after the crash. Witness Elizabeth Keegan stated that she was driving in her vehicle south bound on Lexington St two vehicle behind Wint's vehicle. Keegan stated that she was not sure what color the traffic light was at the time of the crash, but traffic was stopped both not and south bound traffic was stopped. Keegan also stated that Wint's vehicle " didn't even slow down ". Keegan stated that Wint's vehicle struck the female pedestrian and the dog as they were in the marked crosswalk in front of 277 Lexington St. Witness Henry Taylor stated that his vehicle was the first one stopped at the red crosswalk light at 227 Lexington St in the north bound lane. Taylor stated that north and south bound traffic was stopped and that Wint's vehicle did not stop and stuck a female pedestrian and a dog in the crosswalk. Witness Jacqueline Deos stated that her vehicle was stopped behind Taylor's north bound on Lexington St. Deos stated that north and south bound traffic was stopped for the red light . Deos stated that Wint's vehicle did not stop and that it struck a female pedestrian and a dog in the marked crosswalk. The pedestrian in the crosswalk was identified as Xiaoping Sun. Sun stated that she and her dog stopped on the south bound side of the marked crosswalk across from 227 Lexington St. Sun stated that she activated the crosswalk light and waited until the light was red for north and south bound traffic, and that the walk single was activated. Sun stated that she looked to her left and saw traffic north bound traffic was stopping. Sun stated that she turned to her right and entered the marked crosswalk. Sun stated that she and her dog had crossed over the north bound side of Lexington St and she then looked to her right and saw Wint's vehicle not stopping and " going very fast ". Sun stated that the front driver's side mirror of Wint's vehicle stuck her in the face . Sun stated that as she fell to the ground she was struck on the left leg by Wint's driver's side. Sun stated that she did not lose consciousness after she had fallen to the ground. Sun was transported via EMS to The Brigham and Women's Hospital for treatment for head and neck injuries and abrasions to her legs. Sun's dog died as a result of being struck by Wint's vehicle. Photo's of the crash site and of Wint's vehicle were taken and the disc was downloaded by the NPD's IT Bureau. Before leaving the crash site, I tested the crosswalk lights and they were in working order. Based upon my observations of the crash site, Wint's vehicle, and the statements made to me, I concluded that Wint was at fault for this crash. I issued Wint MA Citation T 2013958 and cited him for a violation of MGL 89/11 , failure to yield to a pedestrian in a crosswalk.

On 02/15/20 at approximately 13:27 Officer Anderson and I responded to #14 Elliot St- The Negoshians Sunoco Gas Station to meet with the operator of Mass. Reg# 7EK387 Grey 2017 Mazda 3, Galina Zavlin. Zalvin stated she spoke with Edward Negoshian who told her she ran into Fuel Pump #6 and dented the metallic base after striking the fuel nozzle of Pump #4 approximately a half hour ago while she was fueling up. Zalvin admitted to hitting Fuel Pump #4 with her passengers side mirror but denied ever striking Fuel Pump #6 or the Oil Dolly Display as stated by Negoshian. Officer Anderson asked Negoshian if his security cameras were working and he pointed to the closest one and said "not that one" . He went on to say that he didn't know how to retrieve the security footage. I observed the Mazda 3 in question and didn't see any damage or dents in the front of the vehicle. The approx 1-2" dent on Fuel Pump #6 is showing signs of rust and appears to be old damage. Employee John O'Donnell stated he saw Zalvin strike Fuel Nozzle #4 but never saw her hit Fuel Pump #6 with her vehicle.

On Friday, March 6, 2020 between approximately 0218 to 0220 hours MV1 was parked at 258 Centre St, Newton. At this time an unknown MV struck MV1 from behind and pushed it approximately 10 feet to the Sunoco Entrance (170 Galen St, Watertown). MV1 sustained heavy rear-end damage and was towed privately by the Sunoco owner with MV1 owner's permission. The Sunoco owners do not have a recording of the crash but has the recording of MV1 being pushed from behind into the camera view. The unknown MV was not seen in the camera recordings. MV1 owner was inside his residence at the time, and no injuries were reported.

On Wednesday, May 6, 2020 while assigned to Traffic unit N525, I responded to the area of Harvard Street and Washington Park, Newton for a report of a construction vehicle that crashed into a City of Newton fire hydrant. The weather at the time of the crash was clear and sunny. The road surface was dry. Harvard Street and Washington Park are both public ways maintained by the City of Newton. I spoke with the operator of the vehicle involved in the crash, Michael Clavette (S36731553). Clavette is employed S and P Construction and was operating a 1998 Mack 700 (MA CON: S71054) while working as a sub contractor for Feeney Brothers on Washington Park. Clavette stated he was attempting to back down Washington Park (W) from Harvard Street. Clavette stated he pulled his vehicle forward to reposition and the front bumper area made contact with a fire hydrant in front of 82 Harvard Street. I observed minor damage to the front bumper area of MV1. I observed minor damage to the fire hydrant. Photos were taken of the damage and submitted to the IT Bureau. The City of Newton Water Department was notified.

On Sunday, May 31st, 2020 at 0145 hours, I responded to 20 Nahanton St for a single motor vehicle collision into a telephone pole. The motor vehicle (Mass Reg #9ATB20 2017 BMW COLOR BLACK) was being operated by Robert Shklyar. Robert stated he was driving north on Dedham St when he took a left hand turn at the intersection of Dedham and Nahanton St. While turning left on Nahanton St, he stated he lost control of the vehicle and drifted into the other lane where he crashed into the telephone pole in front of 20 Nahanton St. Robert also stated the roadway was dark and he did not see the curve in the road ahead of him. I observed all the roadside lights to be working properly upon inspection. Robert did not have any injuries. The motor vehicle suffered major front end damage with air bag deployment. Roadside Rescue responded and Towed the BMW. Eversource and Verizon were notified about the pole. The vehicle is insured by Safeco.

On Sunday July 5, 2020 at approx. 0940 hours I responded to 225 Harvard Cir.for a report of a past hit/run that occurred in the area of 137 Comm Ave between the Saturday July 4, 20202, 2330 hours - Sunday July 5, 2020 at 0800 hours. Upon arrival I spoke with the RP Alison Leav, she stated she parked her vehicle in the area of 137 Comm Ave around 2000 hours on Saturday July 4, 2020. She stated she observed her vehicle again around 2300 hours with no damage, and this morning at approx. 0800 observed the driver side of her vehicle had been side swiped from the rear bumper to the front fender. She also showed me a broken piece of a vehicle that was involved in the crash. I observed damage on the driver side rear quarter/bumper along the whole driver side to the front fender including the both left side wheels. I looked at the broken piece belonging to the other vehicle. The color appeared to be blue and was consistent with the color on the damaged left side of the vehicle. The broken piece appeared to be a right side mirror with a writing inside the part " RH - 75854 >ABS< CAV 4. I Google the part and may come back to a 1997-2005 Chevy Cavalier. I took pictures of the part that will be downloaded at the I.T. Bureau. No injuries and party was advised of the process.

Upon my arrival I observed a city stop sign ahead sign, as well as a sewer vent pipe, smashed and on the ground in front of 297. I spoke with the operator who stated he was traveling westbound on Webster street and lost control of the vehicle due to the wet road conditions and struck the sign and pole. On July 30, 2020, while on patrol in marked unit N525, I responded to a 2 car MVA in front of 380 Watertown Street, Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. Watertown Street is a public way maintained by the City of Newton. The operator of MV1, Joseph Vara (S27929046), stated he was operating his 2006 Ford dump truck (MA CON: V42297) East on Watertown Street. Vara stated as he was traveling past 380 Watertown Street, MV2 attempted to merge into his lane from a parking spot on the roadway it's front driver side crashed into the rear passenger side tire area of his vehicle. Vara reported no injuries. I observed damage to the outer rear passenger tire of MV1. The operator of MV2, Willis Herlihy (S63431207), stated his vehicle, a 2010 Toyota Avalon (MA: AG2122) was parked in front of 380 Watertown Street. Herlihy stated as he was exiting the parking spot, he looked back and did not see any traffic. Herlihy stated as he merged into the travel lane the front passenger side of his vehicle crashed into the rear driver side tire area of MV1. Herlihy reported no injuries. I observed damage to the front end of MV2. MV2 was removed from the roadway by Tody's Towing.

On Sunday 8/2/20 at approximately 701 hours while assigned to marked unit n499 I was dispatched to the rear of the Lifetime Center (300 Boylston Street) for a single vehicle motor vehicle crash. Upon arrival I observed a Black Honda CRV with significant front end damage next to a dumpster in the shoulder of Florence Street behind the Lifetime Center. Dumpster was placedt there for the ongoing construction at the Lifetime Center. I spoke with the operator of the CRV, LYSGAARD Ingrid, who states she was frustrated by all the noise being made by the construction workers. Ingrid states she drove to the site to take pictures of the workers whom she believed to be working in violation of Newton's city noise ordinance. She pulled over to the side of the road to take a picture with her cell phone, but didnt realize her foot came off the brake pedal causing her to strike the dumpster. Fire and Medics also responded to the crash and ingrid signed a patient refusal after being evaluated by the medics Tody's responded and towed the vehicle. I stayed on scene until the vehicle was removed and Ingid's son came to transport her hiome.

On Tuesday, September 1, 2020, while assigned to marked unit N496, I responded to the area of 780 Beacon Street, Newton for a report of a motor vehicle crash involving a bicyclist. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of the vehicle involved in the crash, Dina Hudson (S33833807). Ms. Hudson stated she was sitting in her 2015 white Jeep Grand Cherokee (MA: 727SZ7) that was parked (E) in a metered parking spot in front of 780 Beacon Street. Ms. Hudson stated traffic in the Eastbound lane of Beacon Street to her left was stopped. There was a red light ahead at the intersection and traffic had built up past where her vehicle was parked. Ms. Hudson stated she opened her driver side door while she was looking for her debit card to go to the bank. Ms. Hudson stated at this time a bicyclist was traveling at a high rate of speed around the stopped traffic towards the intersection and crashed into her driver side door. Ms. Hudson stated the operator of the bicycle then traveled further up the roadway and crashed behind a vehicle stopped in traffic. Ms. Hudson stated she was unsure if the bicyclist crashed into the stopped vehicle or if it had landed on the roadway just before it. Ms. Hudson stated the bicyclist then got up from the roadway and began yelling at her that the crash was her fault. I observed a few small fresh scrapes on the outside of the driver side door area. When I initially arrived at the crash location, I observed Officer Lazarakis checking on his well-being of the injured bicyclist, Marc Lind. Lind was observed laying on the sidewalk in front of 780 Beacon Street. After being evaluated by Newton Medics, I spoke with Mr. Lind. Mr. Lind stated he was traveling on his Trek bicycle Eastbound on Beacon Street towards Langley Road. Mr. Lind stated while traveling through the area at this time, traffic in the Eastbound Lane was stopped and there was a red light ahead of him at the intersection. Mr. Lind estimated his speed at this time to be 10-12 miles per hour. Mr. Lind stated as he travelled past the Santander Bank at 780 Beacon Street, the operator of a parked white Jeep to his right abruptly opened their door into traffic. Mr. Lind stated he attempted to avoid the vehicles door and veered to the left. Mr. Lind stated the right end of his handlebar made contact with the door and as he traveled forward and applied his brakes his right side crashed into the rear of a vehicle stopped in traffic ahead of him. Mr. Lind stated he crashed to the roadway on his right side. Mr. Lind was able to move to the side of the roadway and wait for Newton PD, Fire, and Medics to respond. Mr. Lind reported injuries to his right arm, side, and leg area. Mr. Lind signed a patient refusal with Newton Medics and was transported away from the area by his wife with his bicycle. I observed no substantial damage to his Trek bicycle. At the time of the crash, Mr. Lind was wearing a white t-shirt, black shorts, black, shoes, and a gray helmet with sunglasses. I spoke with a witness to the crash, Steve Worthington. Mr. Worthington stated he was stopped in traffic on Beacon Street about 4 vehicles back from the red light. Mr. Worthington stated he looked out of his passenger door mirror and observed a bicyclist "whipping" down Beacon Street in between vehicles parked on the side of the road and vehicles stopped in traffic at the red light. Mr. Worthington estimated the speed of the bicycle to be approximately 20 miles per hour. Mr. Worthington stated the bicyclist had to "lean" by his vehicle to get by. Mr. Worthington stated a vehicle just ahead of him that was parked opened their door and the bicyclist made contact with the door. Mr. Worthington stated the bicyclist then went down in front of the car that was in front of him. Mr. Worthington stated the bicyclist was rolling around on the ground and screaming "It's your fault" at the woman that opened her door Photos were taken of the bicycle and the vehicle involved and submitted to the IT Bureau.

On 09/21/20 at 14:46 hours, I (Traffic Unit # 523) responded to the parking lot of 88 Chestnut Street for a motor vehicle accident involving a pedestrian. The weather at the time was 60 degrees ( localconditions.com) and the sky was clear and sunny. The road surface was dry. The parking lot is a thru way that the public use to enter/exit the lot or park there. On arrival at the same time with the Fire and Medics, I observed a female laying in the middle of the lot facing down on her back. A small red and gray CVS like type shopping carriage was knocked over and resting on it's side in front of the victim's head. The carriage was moved to the side in order to tend to the victim I also observed a black Honda Civic bearing MA reg.# 3741TW that was involved in the accident. The Honda was out of the parking spot resting approximately two feet away from the pedestrian's legs. I then spoke to the pedestrian identified as Darla Gipson in the rear of the ambulance. She sustained an abrasion to her front left shoulder and arm area which I observed. She also complained of left knee and elbow pain. She stated that she walking through the middle of the parking lot pushing the carriage by the door entrance in a westerly direction towards Chestnut Street. She states that she never saw a vehicle backing up and then she was suddenly struck and believes she was pushed underneath the vehicle. She was then transported to Newton Wellesley Hospital by Cataldo Ambulance. I then spoke to the operator of the Honda Civic identified as Linda Burke (MA S70320414). Ms. Burke was parked in the first spot to the right of the walkway that leads to the entrance/exit doorway. I asked her what occurred and she stated that she was backing her vehicle up to exit the parking lot when she looked behind her not seeing anything in sight. She then started backing up and struck the pedestrian without realizing it. She says the driver of a green cab and his female customer who were no longer on scene were yelling at her to stop her vehicle because the pedestrian was underneath it. She then stopped her vehicle. I observed minor scrapes to the entire rear bumper of the vehicle so it is unknown where it actually struck the pedestrian/carriage. I observed no obvious signs of damage to the carriage. It should be noted that Ms. Burke's Honda does not have a back up camera or back up sensors. There are no surveillance cameras to the apartment complex and no witnesses came forward for the accident. As a result of the investigation, Ms. Burke will be issued Uniform Citation # T1271069 for Newton City Ordinance 19-75 Failure To Use Care in Backing. Digital photos were taken by me of the scene.

On Wednesday, October 7th 2020 while in N526 I responded to 791 Walnut Street the Newton Cemetery Corp for a motor vehicle accident involving a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. The accident occurred inside the cemetery on the public way by the entrance. Upon arrival I spoke with the pedestrian identified as Tova Goldring. Tova stated she was walking in the roadway when she was struck by MV1. Tova stated MV1 was turning left onto the road that leads to the exit and when she realized the operator was not stopping she put out both her hands and made contact with the hood of MV1. Tova stated the impact caused her to fall to the ground and roll into a grassy area next to the road. Tova had minor scrapes from the accident and signed a patient refusal. The operator of MV1 identified as Antonio Domenico Ricci was on scene and his primary language was Italian. I was able to contact his daughter, Daniela Boccia who arrived on scene and translated Antonio's statements. Antonio stated he was turning left onto the main road that leads to the exit of the cemetery and had his left signal activated. Antonio states the sun was glaring into his eyes and made it hard for him to see. Antonio stated he took the left turn and never saw the pedestrian. Antonio states he heard a thud on his hood and stopped the vehicle and that is when he realized he had struck a pedestrian. Antonio was not injured and there was no damage to his vehicle. Pictures of the scene and Tova's injuries were taken by Officer Wilson and submitted to the IT Bureau. Daniela took possession of MV1 and drove Antonio to his residence.

On Thursday, October 15, 2020, while assigned to Traffic unit N525, I responded to the Whole Foods parking lot entrance located at 916 Walnut Street, Newton for a report of a motor vehicle/bicycle crash. Walnut Street is a public way maintained by the City of Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with the operator of MV1, Tyrone Powell. Powell stated he was operating his 2016 Toyota Highlander (MA: VTT188) while traveling across Walnut Street (E) from Carthay Circle. Powell stated there was a strong sun glare at this time on his passenger side. Powell stated as he entered the Whole Foods parking lot entrance, the passenger side of his vehicle collided with a passing bicyclist that was traveling northbound on Walnut Street. Powell reported no injuries. I observed damage to the passenger side hood/fender of MV1. I spoke with the bicyclist involved in the crash, Scott Chamberlain. Chamberlain stated he was operating his bicycle on Walnut Street (N) in the marked bicycle lane near the entrance to the Whole Foods located at 916 Walnut Street. Chamberlain stated he had a bicycle helmet on at this time. Chamberlain stated that he observed a vehicle to his left travel across Walnut Street in front of him into the Whole Foods parking lot. Chamberlain stated he was unable to avoid crashing into him. Chamberlain reported minor injuries and signed a patient refusal with Newton Medics. I observed no substantial damage to his red Specialized bicycle. Photos were taken and submitted to the IT Bureau. While on scene, Officers did notice a strong sun glare coming from the Southeast direction of Walnut Street. This glare would have shined on the passenger side area of MV1 as it crossed Walnut Street.

Operator of vehicle one ( 2016 Infinity MA reg 3YD266 ) Miram Qualter, stated on 11/05/2020 at 10:28 hours while driving south bound on Waverley Ave near the intersection of Tremont St " the light was turning " as her vehicle entered the intersection. Both Tremont St and Waverly Ave are public ways in the City of Newton. Qualter stated that she was driving into solar glare and did not see any traffic in the intersection as she proceeded into it. Qualter stated that she believed that she still had the right of way and that traffic on Tremont St had a red light. Qualter stated that she did not see vehicle two until the time of impact . Qualter stated that she was not injured in the crash. Qualter's vehicle had major damage to the front driver's side windshield and front driver's side quarter panel. Operator of vehicle two Keith Ward stated that he was riding his Itraxx Cruze motor scooter west bound on Tremont St near Waverley Ave on 11/05/2020 at 10:28 hours. Ward stated that west bound traffic had a green light and he proceeded into the intersection. Ward stated that as he was approximately halfway into the intersection when vehicle one crashed into the right front of his vehicle. Ward stated that he was ejected from his motorcycle and onto the hood of vehicle one. Ward was wearing a helmet and his head had contact with vehicle one's driver side windshield causing substantial damage. Ward was evaluated by Newton Medic Unit 3 and refused medical treatment. Ward signed a patient refusal form provided by the medics. Ward had visible head lacerations that caused minor blood loss. Witness Jose David stated that he was riding on a motor scooter behind Ward. David stated that west bond traffic on Tremont St had a green light and that vehicle one did not stop at a red light. Based upon statements made to me and my observations of the involved vehicles, I issued Qualter MA Citation T2016100 and cited her for a violation of MGL 89/9 , failure to stop at a red light. I took photos of both vehicles and the disk was downloaded by the NPD's IT Bureau. Both vehicles were towed by Tody's Towing, and I filled out towed motor vehicle forms. During my investigation of this crash I discovered that the motor scooter that Ward was operating is classified as a motorcycle, because it's cylinder capacity exceeded 50cc. Ward's vehicle speedometer indicted that it's top speed was 50 MPH, and it's VIN tag stated it was a motorcycle. I observed that there was no license plate on Ward's vehicle and he stated to me that it was unregistered. A check of the MA RMV had no record of Ward's vehicle and also revealed that Ward's driver's license was suspended on 12/04/2018. It was also revealed from a of the MA RMV data base that witness Jose David's driver's license statue is expired non renewable as of 01/23/2017. David stated to me that he did not have a driver's license. I also discovered that David's motor scooter was classified as a motorcycle, because as with Ward's vehicle it also had a cylinder capacity exceeding 50cc. David stated to me that his vehicle was unregistered. I issued David MA Criminal Application T2015901 and cited him for violations of MGLs 90/10, operation of a motor vehicle with out a license, 90/34j uninsured motor vehicle and 90/9 , unregistered motor vehicle. I issued Ward MA Criminal Application T2015902 and cited him for violations of MGLs 90/23 , operating a motor vehicle after suspension, 90/34j, uninsured motor vehicle and 90/9 unregistered motor vehicle. I filled out an Operating After Suspension Affidavit and faxed it to the MA RMV Driver Control Unit.

On Tuesday, December 01, 2020, while assigned to Traffic unit N525, I responded to the rear parking lot of 200 Boylston Street, Newton for a report of a motor vehicle crash. I spoke with the reporting party and operator of MV1, Sungmi Lee (S54407875). Ms. Lee stated she was operating her 2017 Hyundai Santa Fe (MA: 6KR971) Eastbound through the second floor of the Wegmans parking garage, a public right of way. The area she was traveling through is designated for one was travel with a vehicle ramp to her right that allows access for vehicle to go up or come down from the third floor. Ms. Lee stated MV2 (2018 Jeep Compass, CT: AR52330), traveled down the ramp from the third floor and did not stop at the stop sign at the entrance to the second floor. Ms. Lee stated the front end of MV2 crashed into the passenger side of her vehicle. I observed damage to the passenger side area of MV1. Ms. Lee stated her neck was sore from the crash but declined having EMS respond to evaluate her. Ms. Lee stated both vehicles pulled over to exchange information after the crash. Ms. Lee stated the operator of the other vehicle, Avery Adams, stated to her she was at fault for the crash. Ms. Lee stated she noticed the license Ms. Adams showed her was expired. Ms. Adams left the area after they exchanged information and Ms. Lee contacted Newton PD about Ms. Adams' license status. I queried Ms. Adams' license and registration status through Newton PD Dispatch who informed me both were suspended. I spoke with Ms. Adams via telephone. Ms. Adams stated she was traveling down the ramp to the second floor of the parking garage and came to stop at the stop sign. Ms. Adams stated she began making her turn onto the second floor when MV1 sped up to go around her. Ms. Adams stated the front driver side of her vehicle crashed into the rear passenger side of MV2. Ms. Adams was advised of her license and registration status and was mailed Massachusetts Uniform Citation T0115918 for Chapter 90, Section 23 (Operate MV with a Suspended License) and Chapter 90, Section 23 (Operate MV with a Suspended Registration).

I met with the operator of MV1, Cameron Jacques (S82958796). Jacques stated he was operating a 2014 Mack 600 (MA CON: R46465) southbound on Etsy Farm Road. Jacques stated there were two vehicles parked on opposite sides of the road as he was attempting to continue straight past both vehicles. The rear passenger side tire area of his vehicle made contact with the rear bumper area of the vehicle to his left (MV2). MV2 is a 2010 Lexus RX450 that was parked unoccupied facing northbound in the southbound lane across from 34 Etsy Farm Road. The rear bumper of MV2 was damaged and removed from the vehicle as a result of the crash. The registered owner of MV2, Pine Cone Construction, was advised. Jacques and his passenger Jose Lux de Leon reported no injuries. Photos were taken of the damage to both vehicles and submitted to the IT Bureau.

Mr. Napolitano stated as he was passing the area of 1185 Walnut Street, a black SUV parked to his right attempted to leave it's parking spot and merge into traffic. Mr. Napolitano stated MV1 crashed into the passenger side of his vehicle. I observed minor damage to the middle passenger side of MV2. Mr. Napolitano reported no injuries. Mr. Napolitano worked for the City of Newton DPW. Photos were taken of the damage to both vehicle and submitted to the IT Bureau.

The operator of M/V # 1, Danielle Finklea MA D/L S09835151, states she was eastbound on Washington Street in her 18' Mitsubishi Outlander (MA reg. # 9TM745) and turned right in to the parking lot of Tedeschi's. While pulling in, Finklea misjudged the distance that M/V # 2 was parked in a spot to the left of her. She then struck the right side of M/V # 2's rear bumper which sustained scrape marks to it. The Outlander sustained scrapes to the driver's side front quarter panel area and a broken headlight lens cover to that same side. The other vehicle (M/V # 2) was a 2014 Ford F350 pickup bearing MA Official Reg.# M87258 which is owned by the City of Newton Department of Public Works. At the time, the F350 was parked in a parking spot in front of the store unoccupied. The operator, Robert Mazzarella, stated he saw the F350 shake as it had been struck so he walked over to the other operator involved. No injuries reported. Ofc. Marini took digital photos of the scene and submitted them to the I.T. Bureau.

On Sunday 4/11/2021, at approx 1219hrs, while assigned to N498, I responded to 141 Needham St in Newton for a MVA car into a building. There I observed MV1 in contact with the building used by retail store TJ MAXX. The operator, Beverly Linsky, was already outside the vehicle. I assisted Linsky to a bench where she was evaluated by Fallon medics. Linsky refused medical treatment. She stated that she was "fine". It should be noted that despite the heavy front end damage to MV1 the airbag did not deploy. Linsky states that while attempting to park her vehicle she accidently stepped onto the gas pedal instead of the brakes and drove into the TJ MAXX store building. MV1 was towed from the scene by Tody's towing service. The store manager was notified. Due to the statements made by the operator and witness, I will be submitting an Immediate Threat form to the RMV. See incident report #21015576 Pictures of the accident were submitted to IT.

On Tuesday, April 20, 2021 while assigned to Traffic unit N525, I responded to the area of 1188 Centre Street for a report of a crash involving a Toyota Prius and City of Newton owned vehicle.. Centre Street is a public way maintained by the City of Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. The operator of MV1, Herbert Kepnes (S88523672), stated he was operating his 2008 Toyota Prius (MA: 77VF98) at the time of the crash. Kepnes stated he was attempting to exit the metered parking spot he was park in directly in front of the Brookline Bank entrance on Centre Street. As Kepnes attempted to merge into traffic, his vehicle's front passenger side crashed into the driver side fender/door area of MV2. Kepnes and his passenger, Sally Kepnes, reported no injuries. I observed moderate damage to the front passenger side of MV1. The operator of MV2, Stephen Dempsey, was operating a City of Newton owned Ford Expedition (MA MVN: 505B) South on Centre Street near the Brookline Bank entrance in his official capacity as a Newton Police Officer. Sgt. Dempsey stated has he was traveling on Centre Street, a vehicle to his left exited a metered parking spot and attempted to merge into traffic in front of him. Sgt. Dempsey was unable to avoid making contact with MV1. MV1 crashed into the front driver side fender/door area of MV2. I observed damage to the front drive side door/fender area. Sgt. Dempsey reported no injuries. Photos were taken of the vehicles and submitted to the IT Bureau.

On Friday, May 7, 2021, while assigned to Traffic unit N525, I responded to the area of Colby Road for a report of a crash involving a City of Newton owned vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with Mr. Michael Sanford from the City of Newton Water Department. Mr. Sanford stated his City of Newton owned vehicle (2013 Ford F-250 (MA MVN: M88493) was parked unoccupied (S) in front of 497 Centre Street, Newton. Mr. Sanford stated his emergency lights were on while he was marking out a service. Mr. Sanford stated he then observed a white 2005 Ford Ecowagon (MA CON: L61558) travel past his vehicle South on Centre Street and make contact with his driver side door mirror. Mr. Sanford stated the vehicle continued down Centre Street and he followed it to Rochester Road where is came to a stop. I observed damage to the driver door mirror. I spoke with the operator of MV2, Ms. Jerrie Spencer (S12241050). Ms. Spencer stated she knew she made contact with MV1's driver door mirror, but was trying to make it to her appointment at the Faulkner Hospital and that is why she did not immediately pull over. I observed damage to the front passenger side door mirror. Ms. Spencer was provided with Massachusetts Uniform Citation 994181AA for Chapter 89, Section 4A (Marked Lanes Violation). Centre Street is a public way maintained by the City of Newton. Photos were taken of both vehicles and submitted to the IT Bureau.

On Thursday May 13th, 2021 at approximately 1353 hours myself and several units responded 55 Chapel Street for a motor vehicle that struck a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. Chapel Street is a public way owned and maintained by the City of Newton. Upon arrival Fire and medical personnel were attempting to remove the pedestrian from underneath the vehicle as her right leg was stuck in the undercarriage. Fire and medics were able to successfully free the pedestrian from the vehicle and render medical attention. The pedestrian was identified as Beth Herr ( S58524067) and it was determined that she was also the owner of the vehicle involved. I spoke to witness Joe Shields who stated he was outside his house located at 56 Chapel Street when his attention was brought to a blue SUV that was unoccupied moving backwards down Chapel Street. Joe stated he first witnessed the operator of the motor vehicle pull up to the UPS drop box located on the sidewalk across the street from his house. He stated he saw the female operator step out of the vehicle and drop off a package. Next he saw the vehicle start to roll backwards and the female operator opened the drivers side door and attempted to pull the emergency break. He stated the vehicle overtook her and she fell to the ground with her foot stuck under the car. Joe states he heard her yell and saw her get dragged by the vehicle to the opposite side of the street where the vehicle was stopped by the curb. Joe stated he tried to assist the woman but she was yelling in pain and he did not want to cause more injury so he waited for help to arrive. Witness Robyn Preston stated she was walking on Chapel Street when she saw the blue SUV rolling backwards with no one inside the vehicle. She states she then saw a woman try to stop the vehicle but she got dragged underneath the vehicle to its final resting place. Robyn stated she stayed on scene until help arrived. I was able to speak with Beth inside the ambulance prior to being transported to the hospital. Beth was conscious and alert but in a considerable amount of pain and discomfort. I asked her what happened to which she stated she drove her blue Toyota Rav4 MA REG: 1FHM53 to the UPS drop box located in the area of 55 Chapel Street. She stated she thought she put the vehicle in park but must not have as it started to roll backwards when she got out of the car. She states she tried to stop the car by opening the drivers side door and attempting to pull the emergency break. The car over took her and she was dragged to the ground with her right foot stuck underneath the vehicle. She was dragged across the street where the vehicle was finally stopped by a curb. Beth sustained minor scrapes and road rash on various parts of her body and her right leg was injured. Fallon medics transported her to the Brigham & Womens Hospital to be treated. Her husband was contacted using Beth's cell phone and notified of the accident. Captain Boudreau and I were able to drive her vehicle to her residence at 50 Broadway Street, Newton. Her vehicle was left parked outside her house and her car keys were left with her husband who was inside the residence. The vehicle did not sustain any damage. Pictures of the scene were taken and submitted to the IT Bureau. I will be following up with Beth at the hospital to check on her status. On Friday May 14th at 0900 hours I spoke with Beth on the phone. She stated she is home from the hospital and doing much better. She states she has right leg pain and road rash from the accident.

On Monday, May 17, 2021 I responded to the parking lot entrance of the Avalon at Chestnut Hill (160 Boylston Street, Newton) for a report of a motor vehicle/pedestrian crash. The weather at the time of the crash was clear and sunny. The road surface was dry. Boylston Street is a public way in the City of Newton. I spoke with the operator of MV1, Mr. Joshua Champion (SA1310472). Mr. Champion stated he was operating his 2013 Lincoln MKS (MA: VT41980) while attempting to exit the Avalon parking lot at the Boylston Street (E) off ramp. Mr. Champion stated he was monitoring the traffic to merge and as he started to make his right turn his vehicle crashed into the left side of a pedestrian in the roadway. Mr. Champion stated he did not see the pedestrian as he was making his turn. I observed no damage to the area where the pedestrian made contact with the car. Photos were taken and submitted to the IT Bureau. The injured pedestrian, Mrs. Joan Contompasis, stated she was walking on Boylston Street (W) with her husband. Mrs. Contompasis stated she entered the roadway from off the sidewalk to cross the Avalon parking lot exit and continue on Boylston Street. Mrs. Contompasis stated she believed MV1 had stopped to allow her and her husband to cross. Mrs. Contompasis stated the vehicle then accelerated and it's front passenger side bumper area crashed into her left side. Mrs. Contompasis fell to the roadway. Mrs. Contompasis was transported to Newton Wellesley Hospital for injuries to her right foot and knee. I spoke with a witness to the crash, Mr. Michl Contompasis. Mr.Contompasis stated he was walking with his wife Joan on Boylston Street (W). Mr. Contompasis stated he was walking just ahead of his wife as they crossed the Avalon at Chestnut Hill exit at Rt. 9. Mr. Contompasis stated he observed a black sedan to his left as he crossed. Mr. Contompasis stated he believed the operator was not paying attention as he exited the parking lot and the vehicle accelerated into his wife's left side.

On Thursday May 27th at approximately 0859 hours I responded to 14 Thornton Street for a motor vehicle accident involving a city contracted Waste Management truck. At the time of the accident the weather was sunny and the road surface was dry. Thornton Street is owned and maintained by the City of Newton. Upon arrival I spoke with the operators involved. Operator of MV1, MA REG 31X870 identified as Amado Lebron (S92772420) stated he was stopped behind the Waste Management truck as it was collecting trash. He stated he could not see around the truck to pass so he stayed behind it. Amado stated the truck started to back up and he started beeping to alert the driver of the truck. Amado stated the truck did not stop in time and his vehicle's front end was crushed by the truck. Amado stated he was not injured. The operator of the Waste Management truck was identified as Jose Lopes (S60186065). Jose stated he was picking up trash bins with the truck and started to back up to collect trash. Jose stated if a vehicle is too close behind him there is no way to see it. He stated he heard the beeping and thats when he stopped the truck and noticed he backed into a vehicle. There was no damage to the truck and Jose was not injured. Jose was standing by for his supervisor with the truck. Amado was waiting for a tow and his vehicle was safely off to the side of the road. Photographs were taken and submitted to the IT Bureau.

On Sunday, June 6, 2021 at approximately 18:14hrs I, Officer Daly, met with Diana Seufert in the lobby of the Newton Police Station. Seufert was reporting a accident she was just involved in. Seufert said she was stopped at the lights on Washington Street preparing to take a left and start Eastbound on Washington Street. Seufert said her light turned green and as she started into the intersection that is when a van bearing MA V48443 drove through the red light causing her to hit the side of his vehicle. Seufert said the van was trying to merge onto the Massachusetts Turnpike at this time. Seufert said they both exchanged information and said she came to the Police Station after to file this report. I have tried to contact the operator of MV2 numerous times to get his statements regarding this accident yielding negative results. There were no injuries to anyone invovled and both vehicles were able to drive away from the scene.

On Sunday June 13, 2021 at approximately 1933 hours I was dispatched to 2322 Washington for a two car MVA with no injuries. MV1 operator stated she was operating eastbound on Washington St (a public way) driving the speed limit. MV1 operator stated she observed MV2 on the westbound side of Washington St attempt to turn left into Starbucks (2322 Washington St). MV1 operator stated by the time she saw MV2 cut across in front of her and into her lane, it was too late and MV1 struck the passenger side of MV2. MV2 operator stated the same story. MV2 operator said she was also going the speed limit on Washington St westbound and attempted to turn left into Starbucks but misjudged MV1's speed, causing the MVA. MV1 sustained extensive total front end damage and was towed by Tody's. MV1 operator requested an evaluation by Newton Medics and was transported to Newton-Wellesley Hospital. MV2 sustained extensive and total passenger side damage and was towed by Tody's. MV2 operator reported no injuries.

On Wednesday, June 16, 2021 while assigned to Traffic unit N525, I responded to the front entrance of the Brown Middle School located at 125 Meadowbrook Road in Newton for a single vehicle crash involving a City of Newton contracted school bus. It was also reported by Newton Dispatch that one of the occupants on the bus had suffered an arm injury as a result of the crash. Meadowbrook Road is a public way maintained by the City of Newton. The road surface at the time of the crash was dry. The weather at the time of the crash was clear and sunny. When I arrived, I was updated by Officer Iarossi that the crash involved a City of Newton contracted school bus transporting students from the Williams School (141 Grove Street, Newton). The students were participating in "Step Up" day and were leaving Brown to go back to Williams. Officer Iarossi stated a student on the bus was injured as a result of the crash. Tegan Quirk was transported by Fallon Medics to Newton Wellesley Hospital with a right arm injury. The remaining students that were on the bus reported no injuries to staff at the time and were moved to another bus to be transported back to the Williams School. I asked the Brown staff that was assembled in front of the school to contact the Williams School and ask them to hold the children involved in the crash at the school once they arrived. I made this request so we could make sure there weren't any new injuries reported; and I wanted to inquire if any of the children witnessed the incident. The staff stated they would contact Williams and relay the request. I spoke with the operator of the school bus (MA BUN: BU44536), Mr. John Aubert (S48613114). Mr. Aubert stated he was operating bus #361 while transporting students from Brown Middle School to the Williams School. Mr. Aubert stated as he was preparing to leave Brown, he made an announcement to students on the bus to sit down. Mr. Aubert stated he got the okay to leave from one of the staff members on the bus. Mr. Aubert stated there was another bus parked in front of him prior to leaving. Mr. Aubert stated he began to reverse the bus to create distance from the bus in front of him, and then operated the bus forward to exit from his position on the right side of the road and merge into the travel lane to his left around the bus parked in front of him. Mr. Aubert stated as he merged onto the roadway, he heard screaming from the students on the bus and the rear passenger side of the bus made contact with a brick support column in front of the school positioned along the sidewalks edge next to the roadway. Mr. Aubert stated he exited the bus and observed a student's arm pinned on the rear passenger side of the bus between the side of the bus and the brick column. Mr. Aubert stated he got back in the bus and reversed it until the students arm was free. Mr. Aubert stated at this time, staff helped the injured student. The staff there helped facilitate transportation for the remaining students back to the Williams School. Mr. Aubert reported no injuries, but was concerned for the welfare of the student. After speaking with Mr. Aubert, I observed damage to the school bus and the brick support column as a result of the crash. I observed fresh damage (scrapes) to the rear passenger side of the school bus between the last passenger side window and the back of the bus. I also observed fresh damage to the brick support column. The column is a support to an overhead covering that runs from the sidewalk to the front entrance of the school. There was fresh paint transfer from the bus and a crack in the column. Pictures were taken of the damage and submitted to the IT Bureau. The travel lane where the crash took place is a one way access road that runs from Meadowbrook Road to the front of the school, and exits onto a parking lot between the Brown Middle School and the Oak Hill Middle School. I responded to the Williams School to follow up with staff there regarding this crash. All of the students that were involved in the crash had been released prior to my arrival. I spoke with the Principal of the Williams School, Ms. Ayesha Farag, Ms. Michal Shapiro (Social Worker), and Ms. Katie Costello (School Nurse) about the students on the bus. They stated no injuries were reported to them, and they notified the parents of the involved parties about the crash. They also provided me with contact information for all of the students on the bus at the time of the crash. I spoke with the three teachers that were on the bus at the time of the crash, Ms. Kimberly Andersen, Ms. Tess McLean, and Ms. Katherine O'Leary. All three teachers stated prior to the bus leaving Brown Middle School they accounted for all of the students and were ready to leave to travel back to the Williams School. One of the teachers recalled hearing the bus driver make an announcement to the bus that they were about to leave, but could not recall the exact wording of the announcement. The teachers stated as the bus began to move, they heard screaming from the rear of the bus, looked back, and saw a student's arm stuck outside the window. After realizing the student was injured, they assisted with helping to free the student from his pinned position. They helped the injured student until EMS arrived on scene. They assisted the remaining students by directing them away from the crash and onto another bus for transportation back to Williams. The three teachers reported no injuries as a result of the crash. I responded to the Newton Wellesley Hospital Pediatric Emergency Room to check on the status of the injured student, Tegan Quirk. I spoke with Tegan and his mother, Kathryn Quirk in his room. While attempting to talk to Tegan and his mother, Tegan became very upset and did not want to talk about the specifics of the crash. Out of courtesy for Tegan, I exited the room and spoke with his mother outside the room. I explained to Ms. Quirk the events of the crash, and how her son's arm became injured. I asked Ms. Quirk if she could assist me with faciliating a conversation with Tegan about the events of the crash. I explained to her I understood how traumatic the events of the crash were for her son, but as part of my investigation I would need to find out what led to Tegan's arm being outside the bus window at the time of the crash. While speaking to his mother, Tegan explained that he was waving to a friend with his arm out of the bus window when the bus started to move. Tegan stated his arm hit the column, and became stuck between the bus and the column. Tegan stated he was screaming for the bus to move and that it took forever for the bus to move so he could free his arm. Tegan stated the bus driver gave no warning to the students that the bus was going to start moving. The staff at Newton Wellesley Hospital stated Tegan has a right arm injury. The severity of the injury is unknown at this time due to further examination that needs to take place. I advised Ms. Quirk about the conversations I had with the other parties involved. I spoke with Mr. David Stickney (Head of Facilities) from the Newton Public Schools to try and obtain footage of the crash. Mr. Stickney stated unfortunately there is not footage of the crash from the Brown Middle School or Oak Hill Middle School surveillance cameras. I advised Ms. Regina Moody (Newton Public Schools Transportation Director) about the crash. Ms. Moody was able to show me footage of the incident from the camera inside the bus. In the footage, the bus driver can be heard announcing to the students prior to the bus moving, "Guys, guys, get off the seat and sit correctly". One of the teachers can be seen doing a count of the students. The teacher is heard saying to the student, "Everyone needs to be in a seat and sitting at 3 and 2 and 1 and thank you." Student are observed still standing up. The teacher is then heard saying, "sit down sit down." After the teacher passes the students in the back of the bus, she says, "We are good." to the front of the bus. Students are then observed standing after she passes. As the bus begins to back up, students can be observed standing in the rear of the bus and not sitting down on their seats. As the bus moves forward, a student (Tegan Quirk) can be seen standing on his seat and extending his right arm out of the last window on the passenger side of the bus. Screaming can then be heard as his arm makes contact with a support column on the passenger side of the bus. The bus is stopped, the driver exits the bus to asses what happend, gets back on the bus, and reverses the bus a short distance. The injured students right arm is released and he brings his arm back into the bus through the window. The amount of time between the students arm being pinned against the column to when it is released is approximately 40 seconds. After speaking with all parties involved and reviewing the footage from inside the bus, it appears that Bus #361 was pulling out of it's parked position in front of the Brown Middle School to enter the roadway and return students to the Williams School. A student that was sitting in the last row on the passenger side of the bus extended his right arm out of a window next to his seat to say goodbye to a friend. While his right arm was extended out of the rear passenger side window of the bus , the bus collided with a brick support column affixed to the sidewalk in front of the school. When the bus made contact with the column, the students right arm made contact with a brick support column and became pinned between the column and the passegner side of the bus. After the students arm was freed from it's pinned position, the student was able to bring his arm back into the school bus and was transported to Newton Wellesley Hospital for a right arm injury.

On 06/24/21 at 09:20 hours, I (Traffic Unit 523) was dispatched to 140 Adams Street for a city motor vehicle accident involving a Waste Management truck and a parked motor vehicle. Waste Management of Mass Inc. is contracted through the City of Newton to conduct trash pickup for the residents. The weather at the time was sunny and partly cloudy with road conditions being dry. Adams Street is a public way owned and maintained by the City of Newton. On arrival, I spoke to the operator of the Waste Management Truck identified as Jose Lopes (MA D/L # S60186065). Mr. Lopes was operating (M/V # 1) a 2010 Mack Leu dump truck bearing MA. Commercial Reg. # N81671 at the time. It is a right side loading truck that picks up the trash using a double claw mechanism. Lopes states that he did a trash pickup in the driveway right before 140 Adams Street and proceeded to drive northbound on Adams Street. While doing so, the left side double claw retracted back outward after it was closed by him. The claw then struck parked M/V # 2 which was parked in a legal spot in front of 140 Adams Street. The claw struck the driver's side rear bumper/taillight area and scraped along until the driver's side rear passenger door. Mr. Lopes then stopped the truck after noticing. This caused scrapes and dents to the rear bumper, rear quarter panel, and the rear door to that driver's side area. After, I was showed that the truck's claw feature was having mechanical failure as mentioned above. The claw was manually pushed in and secured. The truck was then put out of service. M/V # 2 is a 2013 Jeep Compass color blue bearing MA reg. # 3055GB. The owner Joseph Cedrone (MA D/L # S50190711) was parked and inside his vehicle at the time of the accident. On Wednesday, August 4, 2021, while assigned to marked unit N493, I responded to the area 2115 Commonwealth Avenue, Newton with several other units for a report of a motor vehicle crash with entrapment. Commonwealth Avenue is a public way maintained by the City of Newton. The weather at the time of the crash was clear and sunny, The road surface was dry. On arrival, I spoke with the operator of MV1, Mr. Robert Bemis (S57110680). Mr. Bemis stated he was operating his 2019 Nissan Murano (MA: 1WHT71) on Commonwealth Avenue (W) towards Melrose Street. Mr. Bemis stated a red SUV traveling on Commonwealth Avenue (E) entered his travel lane and he had to swerve to avoid crashing head on. The front driver side of MV2 crashed into the driver side of MV1. I observed moderate damage to the entire length of MV1's driver side. Mr. Bemis reported no injuries and signed a patient refusal with Fallon Medics. Mr. Bemis was able to operate his vehicle away from the area. I spoke with the operator of MV2, Mr. Robert Shea (S86188377). Mr. Shea stated he was operating his 2011 Chrysler Town and Country (MA: 2LYT84) on Commonwealth Avenue (E) towards Lexington Street. Mr. Shea stated he was attempting to make left turn towards Lexington Street when MV1 hit him. Mr. Shea was transported to Newton Wellesely Hospital for evaluation. I observed signifcant damage to the front driver side of MV2. MV2 was removed from the roadway by Tody's Towing. The point of impact of the crash occured in the Westbound lane of Commonwealth Avenue. While traveling Eastbound on Commonwealth Avenue, MV2 crossed the double yellow unprotected median on the roadway and entered the Westbound travel lane. MV2 applied his brakes leaving skid marks to the point of impact and crashed into the driver side of MV1. The operator of MV2 will be cited with Massachusetts Uniform Citation 090421AB for Chapter 89, Section 4a (Marked Lanes Violation).

MV#1 WAS FLOODED INSIDE AND WITH THE CURRENT IT WAS PUSHED UNDER THE FOOT BRIDGE WHICH CAUSED MAJOR DAMAGE TO IT. TRAFFIC RESPONDED AND TOOK PICTURES. On Sunday, August 22, 2021, I was contacted by Sgt. M. Wade of the Newton Police Traffic Bureau at 0032 hours. Sgt. Wade stated a serious motor vehicle crash occurred on Washington Street in Newton and it involved a City of Newton marked police cruiser (N494). Sgt. Wade stated Officer Katelyn Pohlman and the operator of the other vehicle involved were transported to the hospital with potential serious injuries. I was requested by Sgt. Wade to respond to the scene from my residence and investigate the crash further. On arrival at the scene, I observed the two vehicles involved in the crash to be a 2015 Ford Explorer (MA: 586RT7) and Newton Police marked unit N494 (MA MVN: MP494B). These vehicles were observed disabled on the roadway in front of 612 Washington Street, Newton. There was a large debris field comprised of various parts from both vehicles on the roadway and in the parking lot of 612 Washington Street. The road surface at the time of my arrival was wet and there was light rain falling. Sgt. Wade reported that at the time of the crash the road was completely dry and it had not started to rain yet. Washington Street is a public way maintained by the City of Newton. The portion of Washington Street where the crash took place supports two- way travel, Eastbound and Westbound. There are two travel lanes in both directions. Travel direction is divided by a double yellow line in the middle of the roadway. The area is well lit and features numerous light poles illuminating the roadway. The closest intersection to this location is Crafts Street just west of the crash location. MV1 (MA: 586RT7) had substantial front-end damage, especially to its front passenger side and was inoperable. I observed this vehicle at its final rest on the roadway with its front end facing Northbound on Washington Street just over the double yellow line in the middle of the roadway. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV1 was removed from the roadway by Tody's Towing. MV2 (MA MVN: MP494B) had substantial front end damage and was inoperable. The impact from the collision caused the vehicle to spin uncontrolled and the final rest, with the front end of the vehicle facing the front entrance to the 7-11 (S). The vehicle's final rest was on the sidewalk in front of 612 Washington Street. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV2 was removed from the roadway by Tody's Towing. The Officer involved in the crash is Officer Katelyn Pohlman. Officer Pohlman stated she was operating Newton Police marked unit N494 on Washington Street (E) past Crafts Street. Officer Pohlman stated as she was operating her vehicle past 612 Washington Street (7-11), a vehicle in the westbound lane of Washington Street abruptly crossed the double yellow line in the roadway and entered her travel lane head-on. Officer Pohlman stated she did not have an opportunity to react to the vehicle in front of her and the front passenger side of MV1 crashed into the front end of her vehicle (MV2). Officer Pohlman sustained injuries to her left forehead area and both of her arms. Officer Pohlman was later transported to Brigham and Women's Hospital for evaluation and was released a short time later. It was reported to me by numerous Officers on scene that Officer Pohlman exited her cruiser after the crash and rather than tend to her own injuries, she focused her efforts on rendering aid to the operator of the other vehicle involved. Officer Pohlman found the operator of the other vehicle lying on the floor of the front passenger seat under the dash. Officer Pohlman rendered aid to the operator while waiting for Newton Medics to arrive. Officer Pohlman was also assisted in rendering aid to the other operator by Officer Conary and Officer Larson. Officer Larson reported that while interacting with the operator of MV1, the operator repeated numerous times, "Is she ok? Is she ok?". Officer Larson also stated the operator said to him "I feel bad because I was texting on my phone". Officer Larson stated the operator of MV1 works for the Department of Corrections Special Operations Unit and has Department issues equipment inside the vehicle. The following items were placed into Support Services, locker 9 for safekeeping by Officer Larson: 1 Gray Pelican Gun Case, 1 Black Gun Belt, 1 Taser, 1 Can OC Spray, 15 (50) Round Cases Of Ammunition. The operator of MV1 (MA: 586RT7) was identified as Mr. David Cafarelli (S16965940). Mr. Cafarelli was transported to Newton Wellesley Hospital for a potential head injury before my arrival. I spoke with Mr. Cafarelli at Newton Wellesley Hospital in the Emergency Room with Sgt. Wade. Mr. Cafarelli's mother and father were present when we spoke with him. Mr. Cafarelli stated he was out with friends before the crash and was driving to the 7-11 on Washington Street. Mr. Cafarelli admitted that before the crash, he had his cell phone in his hand while he was operating his vehicle. Mr. Cafarelli stated he does not remember if he was texting or changing the music on his phone at the time of the crash. Mr. Cafarelli stated he looked up and was unable to avoid the vehicle (N494) directly in front of him. Mr. Cafarelli stated he also does not believe he applied his brakes before the collision. When we spoke with Mr. Cafarelli in the Emergency Room, he was still waiting for test results to determine the severity of his injuries. On Monday, August 23, 2021, Sgt. Wade responded to 612 Washington Street, Newton to retrieve surveillance footage from the 7-11 of the crash. 7-11 stated their outside surveillance system was not operating correctly, but they were able to capture some of the incident from a camera inside the store. I observed this footage. MV1 is observed traveling on Washington Street (W) in the westbound travel lane in front of the storefront. MV1's driver side is then observed traveling over the double yellow line in the middle of the roadway and into the eastbound lane of Washington Street. A head on collision ensues and MV1 can be seen rotating counterclockwise in the roadway backwards to its final rest location on Washington Street. MV2 is then observed at its final rest position on the sidewalk facing southbound in front of the 7-11 storefront. After completing my investigation, I have surmised the following. Mr. Cafarelli was operating his 2015 Ford Explorer (MA: 586RT7) in the area of 612 Washington Street, Newton. Mr. Cafarelli was holding his cell phone in his hand while operating his vehicle. Mr. Cafarelli became distracted while he was operating his vehicle due to his attention being focused on his cell phone. Mr. Cafarelli traveled over the double yellow line from the westbound lane of Washington Street and entered the eastbound lane of Washington Street. Newton Police marked unit N494 was traveling on Washington Street (E) in the area of 612 Washington Street at this time. Mr. Cafarelli's vehicle crashed into the front end of Newton Police marked unit N494. Mr. Cafarelli was not wearing a seatbelt and as a result of the impact from the crash, he was ejected from the driver seat and his final rest inside the vehicle was in the flood of the front passenger seat. As a result of my investigation, Mr. Cafarelli was issued Massachusetts Uniform Citation for Chapter 90, Section 13B (Hands-Free Violation), Chapter 89, Section 4a (Marked Lanes Violation), and Chapter 90, Section 13a (Seat Belt Violation). Photos from the crash scene and the surveillance media from 7-11 capturing the crash were submitted to the IT Bureau.

Ms. Doucette was operating this vehicle in her official capacity of a Newton Parking Control Officer. Ms. Doucette stated as she was traveling past 82 Lincoln Street, MV2 attempted to exit their parking spot on the right side of the road and merge into MV1's travel lane. MV2's front driver side bumper/fender area crashed into the middle passenger side of MV2. The operator of MV1 reported no injuries. The operator of MV2, Nancy Greenwald, stated her 2012 Honda Accord (MA: 2EGT90) was parked in a parking spot in front of 82 Lincoln Street. Ms. Greenwald stated while operating her vehicle she attempted to merge onto Lincoln Street (E) from her parking spot. Ms. Greenwald stated as she made her left turn into traffic, the front driver side of her vehicle crashed into the passenger side of MV2 as it passed by. Ms. Greenwald reported no injuries. The operator of MV2, Mr. Denis Doucet (S15405545), stated to Officer Durickas he was operating a City of Newton owned 2018 Ford F350 (MA MVN: M2954A near 277 Elliot Street (N). Mr. Doucet stated the vehicle in front of him came to a quick stop and the front end of his vehicle crashed into the rear of MV1. Mr. Doucet reported no injuries. I spoke with the operator of the vehicle involved, Mr. Thomas Mogan (S29907615). Mr. Mogan stated he was operating his 2015 Jeep Wrangler (MA: 6EBH20) on Beacon Street (W) towards his residence of 358 Beacon Street. Mr. Mogan stated he observed a break in the traffic traveling on Beacon Street (E) towards Hammond Street in front of his driveway. Mr. Mogan took a left turn from the Westbound lane of Beacon Street to enter his driveway. While making his turn, his vehicle crashed into a bicyclist traveling on Beacon Street (E) in the marked bicycle lane in front of 358 Beacon Street. Mr. Mogan stated he did not see the bicyclist until the front passenger side fender/door area of his vehicle made contact with her. I observed minor damage to this area on his vehicle. Mr. Mogan reported no injuries. The injured bicyclist was identified as Ms. Louisa Palmer (S67185637). Ms. Palmer was treated on scene by Fallon Medics for injuries to her left wrist and ankle and transported to Brigham and Women's Hospital for further evaluation. Ms. Palmer was wearing a helmet at the time of the crash and I observed her blue Bianchi Pista bicycle resting on the sidewalk in front of 358 Beacon Street. The bicycle had minor damage in to it's front wheel area and fresh scrapes near the handlebars. I transported the bicycle to Ms. Palmer's residence. I responded to Brigham and Women's Hospital in Boston and spoke with Ms. Palmer in the Emergency Department. Ms. Palmer stated she was riding her bicycle on Beacon Street (E) towards Boston College. Ms. Palmer stated she was riding in the marked bicycle lane with the motor vehicle traffic to her left. Ms. Palmer stated she observed a break in the traffic on Beacon Street ahead of her, and then a Jeep entered her travel lane after it took a left turn from the Westbound travel lane on Beacon Street right in front of her. Ms. Palmer stated her left side crashed into the front passenger side wheel/hood area. Ms. Palmer stated she then fell to the roadway. Ms. Palmer stated she has injuries to her left wrist/arm area, her left ankle, and her hips. I spoke with a witness to the crash, Ms. Kelli McCusker. Ms. McCusker stated she was traveling on Beacon Street (E) towards Boston College at the time of the crash. Ms. McCusker stated there was heavy traffic in the area as she observed a bicyclist travel by the passenger side of her car. Ms. McCusker stated there was a break in the traffic ahead of her in the Eastbound lane and she observed a Jeep in the Westbound lane of Beacon Street make a left turn in front of the car in front of her. Ms. McCusker stated the Jeep and the bicyclist crashed into each other at this time. After speaking with all parties involved, Mr. Mogan did not yield to the right of way while Ms. Palmer was traveling on Beacon Street (E) on her bicycle. Mr. Mogan will be issued Massachusetts Uniform Citation for Chapter 90, Section 14B (Fail To Yield To Bicyclist on Turn). Photos were taken of the scene and submitted to the IT Bureau.

On Sunday, October 10, 2021, at approximately 1601 hrs, while assigned to N494, I responded to 320 Washington St., the Four Points Sheraton, (rear Centre St. side) for a single motorcycle accident ( MV1- MA Reg 2S2136). Upon arrival, the fire and medics were treating the operator of the motorcycle, identified as BASS, Braxton Lee. BASS stated he came off the Mass Pike Eastbound exit, traveled three lanes to very left lane on the Washington St. rotary (EB), and he hit a pot hole in the road. He stated he then lost control of the motorcycle and was thrown over the handlebars crashing onto the sidewalk. The motorcycle skidded the length of the curb until the stair area of the hotel. BASS sustained injuries to his left shoulder, hands, left torso, knees, and the left side of his head. He was transported by Medic 5 to the BI Boston. The motorcycle sustained minor damage and was towed from the scene by Todys. I observed a dip in the left lane where BASS and witness on scene stated he initially lost control of the motorcycle. See attatched photos. Witness statement: MAZZIE, Zach stated he witnessed BASS crash his motorcycle. He stated BASS came off the Mass Pike exit, traveling Eastbound on Washington St., when he proceeded to the left lane, and was thrown from the motorcycle. On 10/19/21 at 08:30 hours, Sgt. Wade notified me that he received an email regarding a motor vehicle accident involving a pedestrian/student that occurred on 10/15/21 between the hours of 08:20am and 08:35am. The crash was never reported by the operator of the vehicle or the student/pedestrian. Approximately two hours later, the student went to the nurse's office seeking assistance for neck pain which is how the school became aware. This crash occurred in the marked crosswalk on Brandeis Road in front of Newton South High School which is located at 139 Brandeis Road. The weather at the time was 63 degrees with mostly cloudy and sunny conditions (wunderground.com). Brandeis Road is a two way roadway divided by an unprotected median that runs east and westbound. In front of the school there is a marked crosswalk in the roadway. There is also a one way driveway near the main entrance of the school with parking stalls and parents can drop their children off there on the right side of the driveway. The driveway then leads back out to Brandeis Road. Right after you exit the lot there is the above mentioned crosswalk to the left of the driveway which is very visible with nothing obstructing your view. On arrival, I spoke to the Dean of the school named Charlie Myette. Mr. Myette brought me to his office and started looking at the school's video surveillance system. He also requested that the student/pedestrian involved to report to his office. The student was identified as Tsukasa Iinishi. I asked Mr. Iinishi what had occurred and he stated that he was walking to school on the morning of 10/15/21. He says that he was wearing a pink hoody sweatshirt with gray shorts. He was walking through the woods which is across the street from the school which exit out to Brandeis Road. He says that he waited at the crosswalk and let two vehicles go westbound by him. He then thought it was clear to proceed and started walking. At the tail end of the crosswalk he was struck by a red colored Toyota Prius which exited left from the driveway and was on the left side of the roadway. He was struck on the left side of his body and he says that he used his left hand to brace himself against the hood and that's when the Prius stopped. He says that the operator of the Prius asked him if he was okay and he replied that he was. He then kept walking toward the school. The operator then continued on it's way without providing any information or notifying us. He described the operator as an Asian female with black hair approximately 40 to 50 yrs in age. During this time, Mr. Myette was able to find surveillance video of the crash. There are two camera angles. One shows the driveway along with the walkway of the school building. The other one shows the same but it also overlooks out in to Brandeis Road. The view of the road is partially blocked from the parked cars in the lot. The video shows a red colored Toyota Prius pull in to the driveway of the school. An Asian female student wearing a white long sleeve shirt with green lettering and dark colored jeans exits the passenger front of the Prius and eventually enters the school. The Prius then pulls out and goes toward the left side of the driveway around other vehicles waiting to exit. At 08:27 hours, the Prius pulls out angling towards the left side of the driveway going around other vehicles waiting to exit. The Prius then exits the lot on the left side (wrong side) of the roadway and makes contact with the pedestrian. In the video you cannot see the front hood area of the car, but you see Iinishi's torso bend towards the vehicle on impact. It shows the Prius stopped there for approximately ten seconds and then it drives away. In the video you are not able to see the registration plate number. Mr. Myette is going to send the photo of the student dropped off in the Prius to his staff to have her identified so I can follow up with the operator. On 10/21/21, I received an email from Mr. Myette stating that some of his staff has not come forward on the identification of the student. He stated that he would now send a message out to a larger group of staff to see if he gets a response. He also attempted to enhance the registration plate using the surveillance software but the plate is still too blurry to make out. On 10/25/21 at approximately 08:40 hours, I went over to Newton South High School to see if I could locate the red Toyota Prius involved in the crash. As I was driving on Olde Field Road approaching Brandeis Road near the school, I observed a red Prius approximately four cars ahead of me in heavy school traffic. Eventually, I observed the Prius pull in to the drop off driveway of the school and drop an Asian female student off. I identified her as the same student that got out of Prius prior to the past accident on 10/15/21. The operator was an Asian female which appeared to be in her late forties. After, the Prius exits the lot on to Brandeis Road and I flagged her down. I had her pull in to the parking lot across the street from the school. I asked her if she was involved in a accident with a student on the morning of 10/15/21 to which she replied that she was. She was identified as Xiaojin Liu through her MA Driver's License # S49033927. She was operating a 2010 Toyota Prius bearing MA reg.# 8JY630. I asked her what occurred and she stated that she was exiting the driveway of the school to turn left on to Brandeis Road. She says that she has a habit to think that no one is ever walking through the crosswalk and thought it was clear to proceed. She turned and then saw the student crossing and attempted to stop but was not able to in time. She then bumped the student with the front of her vehicle. She says that she stopped and asked the student if he was ok several times and he stated that he was. The student then walked away and she says that she did not know what else to do. I advised her that she should of gave her information to the student, notify the school, or call us to which she understood. I issued MA Uniform Citation # 172129AB to Ms. Liu for Ch. 90/24/A Leaving the scene of personal injury. After, I went inside the school and notified Mr. Myette of my findings. I asked him for the surveillance footage as I previously did in person and through email. He stated that he would put the video on a zip drive and notify me.

On 10/21/21 at 15:24 hours, I (Traffic Unit 523) responded to 1479 Washington Street for a motor vehicle accident involving a bicyclist. The weather at the time was seventy three degrees with mostly cloudy skies and some sun(weather.com). The road conditions were dry and Washington Street is a public way maintained by the City of Newton. Washington Street is a two way divided roadway with an unprotected median that runs in a easterly and westerly direction. At this location, there is a marked crosswalk with a walk signal along with traffic signals above in both directions. On arrival, the scene was cleared up and both parties were waiting in the parking lot of the Burke & Blackington Funeral Home at 1479 Washington Street. I spoke to the bicyclist involved who was identified as Rajmattie Thakoordeen. I asked her what occurred and she stated that she was attempting to cross Washington Street in a southerly direction on her electric bicycle. She says that she pushed for the crosswalk signal to activate and once it did she began riding her bicycle when she was suddenly struck by a silver Saab 93. She made it out in to the crosswalk approximately four feet when she was struck by the Saab which was travelling westbound in the right lane of Washington Street. She says that she was struck on the left side of her leg area. She complained of her left leg being sore near her knee area but signed a patient refusal with Fallon Ambulance declining transport. At the time, she was riding a Rad Power Bike Roadrunner 1 Plus that is silver and black in color. It is a Class 2 electric bicycle (750W) that can go up to 20 m.p.h. The only damage it sustained was scrapes to the outside left pedal. Ms. Thakoordeen was wearing black/gray/white pants, yellow reflective jacket, and a black bicycle helmet at the time. Next, I spoke to the operator of the Saab 93 bearing RI registration # FN193. He was identified as Nevan Hanumara with Rhode Island Driver's License # 9960501. He stated that he was travelling westbound on Washington Street in the right travel lane when he came around the curve and noticed the red light signal above in the last second. He says by the time he reacted and attempted to stop he went through the red light and made contact with the bicyclist in the crosswalk. He reported no injuries and no damage to his vehicle. Due to his statement and inattention to the traffic signal, I issued him MA Uniform Citation # 168985AB for Chapter 89/9 Fail to Stop/Yield for the red traffic signal. Digital photos were taken of the scene.

On 11/03/21 at 08:25 hours, N491 and I (Traffic Unit # 523) responded to 612 Washington Street for a two car motor vehicle accident involving a city motor vehicle. The weather at the time was clear and sunny and the road conditions were dry. Washington Street is a public way maintained and owned by the City of Newton. On arrival, I spoke to the operator identified as Brian Napolitano (MA D/L #S73156918) and his co-worker Kevin Brooks who was a passenger in the vehicle. Both parties work for the City of Newton Department of Public Works. Mr. Napolitano was operating a 2009 Ford F350 bearing MA MVN registration# M78129 at the time. He stated that he was attempting to back out of the driveway of 612 Washington Street. Once it was clear to proceed he backed out on to Washington Street and that is when a Subaru Impreza made contact with his driver's side front quarter panel area. He says the Subaru was making a left turn from the left westbound lane of Washington Street in to the same driveway he was backing out of at the time. There was no visible damage to the vehicle. I then spoke to the operator of the Subaru (MA reg.# 3GYT41) identified as Jennifer Shippie (MA D/L# S23299202). Ms. Shippie states she was in the left westbound lane of Washington Street and was turning left in to the driveway of 612 Washington Street. She further stated that a vehicle in the eastbound lane stopped for her and waved her on to proceed. She then made the turn and never saw the Ford F350 backing out and made contact with it on her passenger front bumper area. The entire vehicle had damage to the front bumper consisting of scratches and dents but she stated none of it was new from this accident. No injuries reported and digital photos taken of the vehicles by me.

On Friday November 5th 2021, at approximately 1304 hours I responded to the area of 36 Greylock Road for a two car motor vehicle accident involving a city contracted Waste Management receptacle truck. At the time of the accident the weather was sunny and the road conditions were dry. Greylock Road is owned and maintained by the City of Newton. Upon arrival I spoke with the operator of the garbage truck identified by RI License 9898223, as Norbin Cordon. Norbin stated he was operating the garbage truck and travelling westbound on Greylock Rd. Due to cars parked on both sides of the road he stopped to let another vehicle go by him coming from the opposite direction. The other vehicle stopped at the bend to let the garbage truck go by. Norbin stated it was a tight squeeze and when he tried to make his way through his rear drivers side tires made contact with the drivers side panel of the other vehicle. The only damage to the garbage truck was a smudge on the tire. Norbin reported no injuries. I then spoke with the operator of the other vehicle involved, identified by Mass Lic as Jane Lewis S79838916. Jane stated she was traveling eastbound on Greylock Road and stopped in the road to let the garbage truck pass. As the truck attempted to pass it struck her rear drivers side panel causing moderate body damage. Jane reported no injuries. Several photographs of both vehicles were taken and submitted to the IT Bureau. Jane's vehicle was able to drive and she left without further incident.

Michael stated he was travelling eastbound on Webster Street when the other vehicle involved backed out of the driveway and struck the front passenger side bumper of the truck. Michael reported no injuries and the vehicle was drive able. I then spoke with the operator of other vehicle involved, identified by Mass permit license as Sophia Mercado SA5660670. Sophia stated she was backing out the driveway and stopped, and that is when the DPW struck her rear end. Sophia reported no injuries and minor damage to the vehicle was reported. On 11/08/21 at 14:08 hours while on routine patrol I came across a two car motor vehicle crash in the westbound lane of 667 Commonwealth Avenue. Commonwealth Avenue is a public way owned and maintained by the City of Newton. Both operators were outside of their vehicles at the time and reported no injuries. I observed Vehicle # 1 (Honda Civic MA reg. # 2CWA44) to have heavy front end damage to the bumper and hood area. It was parked back approximately ten feet from Vehicle # 2. Due to the damage and it leaking anti-freeze coolant, Tody's Towing was notified to tow the vehicle away. Vehicle # 2 (Ford EcoSport MA reg. # 2HJV37) was parked in front of Vehicle # 1 and sustained damage to the center and right rear bumper area. It was still driveable. I spoke to the operator of Vehicle # 2 (Maria Bianchi MA D/L # S97972578) and asked her what occurred. She stated that she was traveling westbound on Commonwealth Avenue in her Ford EcoSport (MA # 2HJV37) but was stopped at the time waiting for a vehicle in front of her to make a left turn on to Grafton Street. She was then struck in her rear end by Vehicle # 1. After, I spoke to the operator of Vehicle # 1 (Maria Kuznetsova MA D/L # S48191231) and asked her what occurred. She stated that she was traveling westbound on Commonwealth Avenue in her Honda Civic (MA # 2CWA44) and did not see Vehicle # 2 stopped in traffic due to the sun. She then struck Vehicle # 2 in the rear end. At that time, there was noticeable solar glare in that area of Commonwealth Avenue. This part of Commonwealth Avenue also has a slight curve that you come around and ends at the point where the crash occurred. After, I went back to my cruiser and checked the status of the vehicles and operators through the RMV on my laptop. It showed that Maria Kuznetsova had a expired non renewable driver's license which expired on 07/15/21. Kuznetsova's Honda Civic came back with a status of being unregistered due to revoked insurance. It was revoked on 08/29/21 through the MA RMV. I issued MA Uniform Citation # 185998AB in hand to her for Ch. 90/10/A Unlicensed Operation of a Motor Vehicle, Ch. 90/9/B Unregistered Motor Vehicle, and Ch. 90/34J Uninsured Motor Vehicle and explained the process to her. Motor Vehicle Inventory Tow Form submitted.

On Sunday November 21st at approx. 1720 Hrs. I responded to 191 Jackson Rd for a motor vehicle accident roll over with entrapment. Upon arrival I witnessed MV1 on its side in the middle of Jackson Rd. The operator of MV1 was conscious and alert and was taken out of the MV by the Newton Fire Department. The operator of MV1 stated that she was driving S/B on Jackson Rd. and she did not see the parked car on the right side of the road. MV1 then tried to swerve around MV2 and clipped the left rear bumper, and left rear door of MV2 which caused MV1 to roll over onto the middle of the road. MV1 had major damage all around the vehicle and all airbags deployed. MV2 had major damage to the rear and to the left side of the MV. Due to the extensive damage and length of travel of MV1 it was estimated that MV1 was traveling at a speed greater than 35 MPH. Speed limit for Jackson Rd is 25 MPH. MV2 was pushed approx. 20 feet from where it was parked, and MV1 travelled approx 25 feet after on its drivers side after impact. The operator of MV1 was transported by EMS to NWH, and Todys's took possession of both motor vehicles. Pictures were taken and submitted to IT. The operator of MV1 was cited 90/17, speeding.

Washington Street is a public way maintained by the City of Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. The operator of MV1, Mr. Michael Leone (SA4940776), stated he was operating his 2013 Dodge Challenger (MA: 2HZW73) on Washington Street (E) towards Armory Street. Mr. Leone stated a vehicle parked on the right side of the street pulled out from it's parking spot to merge into traffic and crashed into the front passenger side of this vehicle. Mr. Leone and his passenger reported no injuries. I observed damage to the front passenger bumper/fender/wheel/door area of MV1. The operator of MV2, Mr. Chad Natichioni (AZ: D08314037), stated he was operating a 2020 Nissan Rogue (MA:7BH214) at the time of the crash. Mr. Natichioni stated he was parked across from 1149 Washington Street in the Eastbound parking area. Mr. Natichioni stated when he attempted to merge into the Eastbound travel lane of Washington Street, the front driver side area of his vehicle crashed into MV1 as it passed. MV1 had the right of way in the travel lane at the time of the crash. Mr. Natichioni apologized and stated he was at fault for the crash. I observed damage to the entire front end of MV2. Tody's towing responded and removed the vehicle from the roadway. No injuries were reported by Mr. Natichioni and his passengers. As a result of the collision, Mr. Natichioni was cited with Massachusetts Uniform Citation 203616AB for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care).

On Sunday 1/2/22 at approximately 1054 hours while assigned to marked unit n498 I was dispatched to the area of 1210 Chestnut Street for a report of a past Hit and Run. Upon arrival I spoke with the damaged vehicle's owner, ODVARKA Joseph, who states last night he parked his vehicle around 4:30pm in his parking spot that runs along a fence bordering the parking lot of Brickfire Pizza and Allstar Liquors (1214 - 1220 Chestnut Street). ODVARKA states between the hours of 4:30pm and 8:30pm last night someone struck the fence on the other side causing it to buckle and do damage to the passenger side of ODVARKA's vehicle. After speaking with ODVARKA I canvassed the area and noticed surveillance cameras at Brickfire Pizza and All Star Liquors. The owner of Brickfire states to the camera only captures footage from the front door and inside the establishment. The owner of All Star Liquor states his cameras only capture footage from the front of the store and the inside and the cameras are real time only and do not record. Businesses across the street were not open and I did not observe any external cameras.

MV1 operator states she was driving WB on Beacon St when MV2 came to a sudden stop and due to the sun's glare, did not see MV2 very well. MV1 operator then rear-ended MV2. I did not observe any damage to the front of MV1 or the rear of MV2. MV2 operator states she was driving WB on Beacon St when MV1 suddenly collided with her rear end. MV2 subsequently rear-ended MV3. MV3 operator states he was stopped, waiting for the cars to go since the light had just turned green, when he was suddenly rear-ended by MV2. MV3 subsequently rear-ended MV4. On Sunday 2/13/22 at approximately 1851 hours while assigned to marked unit n501 patrolling division n496 I was dispatched to the area of 935 Walnut Street. Upon arrival I spoke with the passenger, DELUCA Joseph, who states he was the passenger in Ma Reg 1ANK92 and his girlfriend, BISSANTI Alicia, was the operator. They were travelling northbound on Walnut Street and approaching their home at 935 Walnut Street when BISSANTI activated her left turn signal and slowed to make a left turn into their driveway when they were struck in the rear by a white mini van. The white mini van did not stop and continued Northbound on Walnut Street. BISSANTI tried to catch up to the van and thought it turned into the parking lot of Dominos Pizza. They confronted a white male in the parking lot who was driving a white mini van, however the operator stated it was not him. While at 935 Walnut Street I observed their motor vehicle parked in the back and there wasn't any damage to the car.

Operator of MV1 (MA reg 2RBG35) was traveling EB on Pearl Street when she stopped for a vehicle pulling out of a driveway. While stopped the Operator of MV2 (MA reg 2YDP98) also traveling EB on Pearl Street rear ended MV1. Operator of MV2 stated the solar glare obstructed her vision and by the time she observed MV1 stopped it was too late. MV1 sustained minor damage to the rear of the vehicle and was able to drive away safely. MV2 sustained heavy front end damage and was leaking fluid. Air bags were also deployed from MV2. Medic 5 arrived on scene for an evaluation. Operator of MV2 was claiming a possible head injury and signed a medical refusal. Tody's arrived on scene and towed the vehicle.

On Wednesday, February 23, 2022, while assigned to Traffic unit N525, I responded to the intersection of Washington Street and Elm Street, Newton with N492 for a report of a minor motor vehicle crash. Washington Street and Elm Street are both public ways maintained by the City of Newton. The weather at the time of the crash was clear and sunny. The road surface at the time of the crash was dry. I spoke with the operator of MV1, Mr. Kumar Bhol (S19441804). Mr. Bhol stated his vehicle (2018 Toyota Rav-4 (MA: 8HH159) was parked unoccupied on Elm Street next to the Village Bank in a metered spot. Mr. Bhol stated when he returned to his vehicle after leaving the gym, he observed another vehicle illegally parked behind his vehicle with it's front bumper very close to the rear of his vehicle. Mr. Bhol stated he observed minor fresh damage to the bumper area of his vehicle at this time. Mr. Bhol stated the owner of MV2 returned to her vehicle at this time and he attempted to exchange information with her. Mr. Bhol stated the owner did not want to exchange information and attempted to leave the area. Mr. Bhol stated he positioned himself in front of her vehicle in the roadway as it was leaving to prevent MV2 from leaving. Mr. Bhol stated at this time MV2 made light contact with his knee. Mr. Bhol stated he has no injuries as a result of the contact. Mr. Bhol declined my request to have him evaluated by Newton Medics. Mr. Bhol was also advised to not position himself in front of a moving vehicle to try and stop them from leaving the area. I observed very minor damage to the rear bumper area of MV1. It is inconclusive if the damage is fresh from this crash. I did observe obvious old damage to a sensor on the rear bumper of MV2 with black tape over it. I spoke with the operator of MV2, Ms. Jennifer North (S41819258). Ms. North stated she parked her 2015 Jeep Grand Cherokee (MA: 2XR747) behind MV1 on Elm Street in what she thought was a legal parking space. Ms. North stated when she returned to her vehicle, the owner of MV1 alleged the damage on his vehicle was caused by her. Ms. North stated she did not make contact with MV1 and attempted to leave the area. Ms. North stated Mr. Bhol would not let her leave so they contacted the Newton Police to assist. A query of Ms. North's license status showed it was expired/non renewable. I advised Ms. North of her license status and stated she could not drive her vehicle away from the area. Ms. North stated she understood and walked to her residence with her two dogs. Ms. North's vehicle was left legally parked in a metered parking spot on Elm Street. I observed no noticeable damage to the front bumper area of MV2. Ms. North report no injuries. As a result of Ms. North's license status she was issued Massachusetts Uniform Citation 306986AB in hand for Chapter 90, Section 10 (Unlicensed Operation) and Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care). It is unclear is the damage to MV1's rear bumper was caused by MV2.

On Tuesday, March 01, 2022 at approximately 1609hrs, I, Officer Fay responded to 1340 Centre St. for a report of a past hit and run. Upon arrival I met with the owner of the vehicles son, William Redmond, who stated on Sunday, February 27, 2022 at approximately 9:30PM he parked his dad's vehicle, a black 2017 Ford Edge (MO plate EEON3z) at the bottom level of the parking garage at 1340 Centre St. William stated that when he returned to use the vehicle on March 01, 2022 at around 3:30PM he noticed a dent in the driver side door. I asked him if anyone else has driven the vehicle within that time, which he replied no and informed me that he did not recall seeing that damage before. Upon closer inspection of the vehicle, I noticed the damage appeared to be cover from dirt and salt from the road, making the damage appear to be old. I also noticed that the parking garage does not have security cameras. At this time it is unknown if the damage is old or if someone had struck his vehicle within the garage between February 27 and March 1st. I informed William that a report would be generated, at which he informed me that he has made his father, Christopher Redmond aware of the damage. The vehicle had minor damage to the door and the vehicle is considered safe to drive and left parked in the parking garage. A vehicle was parked next to William's vehicle, but William stated it was his coworkers and knew the vehicle was not parked next to his vehicle. I walked around his coworkers vehicle, which had no damage.

At approximately 0933HRs on Wednesday, March 02, 2022, I was dispatched to 151 Sargent St. for a report of a two car MVA. Upon viewing the vehicles, I observed MV1 (MA Pass: 1HLE75) parked in front of 163 Sargent St. with damage to its passenger's side (fender, wheel/tire, front and rear doors). And I observed MV2 (MA Pass: 6ELY50) in the driveway of 151 Sargent St. with damage to its rear (tow hitch and bumper). Upon speaking with the operators, both of whom reported no injuries, the operator of MV1 stated they were traveling Eastbound on Sargent St., when MV2 attempted to exit the aforementioned driveway and they two vehicles made contact. The operator of MV2 stated that as they exited said driveway, there was solar glare which came from not only the sun in their eyes, but from sun reflected off of an uninvolved vehicle which passed on the opposite side of the road. Being that they were unable to see MV1 approaching, MV2 continued to travel out of the driveway, backing up Southbound, when the two vehicles made contact. Both operators exchanged information prior to my arrival. MV1 and its operator remained in a safe area on scene to await a tow from AAA and MV2 left under its own power. I cleared without further incident.

On 03/08/22 at 14:57 hours, N494 and I Traffic Unit # 523 responded to the Horace Mann School located at 225 Nevada St. for a two car motor vehicle accident involving one of the operator's being struck. The accident occurred on Nevada St. just past a driveway of the Horace Mann School approaching the intersection of Nevada St and Linwood Ave. The weather at the time was sunny/clear and the road conditions were dry. School dismissal is at 14:50 hours and for approximately ten minutes Nevada Street is very congested with vehicles parked on both sides of the street. On arrival, I spoke to the operator of M/V # 2 identified as James Rice (MA D/L # S83251000). Mr. Rice's vehicle is a 2012 Ford Escape color white bearing MA reg. # 6725ZA. The Ford Escape was parked just past a school driveway facing southbound in a legal parking spot. He says he was picking up his child from the school. He further states that he looked and thought it was clear to open his door. He then opened his door and put his left foot and left arm out attempting to exit the vehicle when it was struck by M/V # 1. M/V # 1 struck the outer edge of the door and Mr. Rice's left elbow was also struck during the collision. Mr. Rice complained of left arm pain so Officer Boudreau had Fallon Ambulance respond to the scene. Rice then signed a patient refusal with them. Also inside the vehicle at the time of the crash was his son, Charger Rice. His son was seated and secured in a car seat in the second row of the driver's side of the Ford Escape. There was no injuries reported for him. I observed a crease to the outside edge of his driver's side door. Mr. Rice also stated to me that the crossing guard told him the other operator was speeding before the crash going approximately 25 miles per hour. Next, I spoke to the operator of M/V # 1 identified as Olga Kulakova (MA D/L# S63003743). She was operating a 2015 Infiniti QX60 color black bearing MA reg. # 5TJ151 at the time. Ms. Kulakova states that she was parked approximately twenty five feet back from the crash location also picking up her daughter from the school. After doing the pickup, she then proceeded out in to roadway going southbound on Nevada St. As she was approaching M/V # 2, she says the operator suddenly opened their door and she was not able to react in time striking his car door. She did not believe she ever struck the operator though. No injuries were reported. I observed white paint scrape marks to both her passenger side doors and the side view mirror was also damaged. I then spoke to the crossing guard assigned to the intersection of Nevada St. and Linwood Ave. The guard Jack Boudreau is employed through the City of Newton. I asked him what had occurred with the crash. He stated that he had his back turned away from the crash location when it occurred. He heard the collision which made him turn around. He then motioned for the driver of the Infiniti to pull over and stop. I asked him if he ever saw the Infiniti driving prior to the crash and he stated that he did not. I then inquired about the supposed speeding comment that he made to Mr. Rice regarding the Infiniti. He stated that he never said that to him. Digital photos were taken of the scene and submitted to the I.T. Bureau.
