VEHICLE #2 STATES SHE WAS TRAVELING N-BOUND AHEAD OF VEHICLE #1. SHE STATES HE WENT UP ON THE SIDEWALK AND LANDED HIS VEHICLE IN FRONT OF HERS. VEHICLE #1 HAD FRONT END AND HOOD DAMAGE, A SMASHED WINDSHIELD AND RIGHT SIDE AIRBAG DEPLOYMENT. THERE WAS NO DAMAGE TO VEHICLE #2. VEHICLE #1 DID NOT MAKE CONTACT WITH VEHICLE #2. MV#1 was traveling north on Chestnut St. MV#1 was going under the Boylston St bridge, and crossing the intersection of the off ramp from Boylston St, when she was struck on her passenger side by a vehicle. Operator of MV#1 states that she did not see the vehicle coming, and that she believes that he ran the stop sign. Operator of MV#1 states that she then parked on the side of the road waiting for the operator of MV#2 to come and speak with her. Operator of MV#1 states that MV#2 drove off without passing paperwork. It should be noted that vehicles traveling north on Chestnut St have the right of was ay this intersection. All other directions have stop signs. Airbags deployed in MV#1, both parties were evaluated by medics, and signed a patient refusal. Both vehicles were driven off scene.

I THAN SPOKE WITH THE OPERATOR OF MA PASSENGER REG 3NM859, THOMAS DORRANCE WHO STATED ' IT APPEARS I'M AT FAULT' DORRANCE STATED HE HAD NOT NOTICED DEVLIN SLOWING AND STOPPING AND RAN INTO THE BACK OF HER. DEVLIN WAS ALSO COMPLAINING OF PAIN FROM THE STEERING WHEEL AND AIRBAG, CATALDO RESPONDED. NFD AND TODYS TOW ARRIVED ON SCENE .NFD USED THE JAWS TO SEPARATE THE TWO VEHICLES TODYS WAS ABLE TO TOW BOTH VEHICLES. DORRANCE SIGNED A PATIENT REFUSAL WITH CATALDO. MV#1 Stated while traveling westbound down Commonwealth ave he struck a patch of ice. The ice caused his vehicle to rotate 360 degrees and proceeded to slide on the roadside shoulder striking a verizon poll ( 102/285). MV#1 traveled approximately 12ft from the road before striking the poll. MV#1 sustained heavy front end damage. MV#1 had all airbags deployed and had a small fire under the front end. I observed multiple other vehicles passing having difficulties and vehicles sliding due to significant ice conditions. Driver of MV#1 had injuries to his left arm, possible broken forearm/wrist. Passenger of MV#1 stated she had chest pains. Medics transported both parties to Newton Wellesley Hospital for further evaluation. Tody's Towed the vehicle due to heavy front end damage.

Narrative:
At approximately 0110HRs on Sunday February 17, 2019, I was dispatched to the area of Dedham St. and Spiers Rd. (both public ways in the City of Newton), for a report of a motor vehicle vs pole accident with injury, airbag deployment and entrapment. Upon arrival, I exited my cruiser with my medical bag and AED and went to assist the motorist involved in the crash. MV1 (MA Passenger: 255ZA9) was completely off the roadway and on the shoulder of the road (facing Eastbound) with its nose contacting the light pole ( Verizon Pole #:103/80), which was now broken and listing to the side. Attempts to open the doors were unsuccessful and I attempted to communicate with the operator through a partially broken driver's window frame which was bent. The operator was slumped forward with his hand cradling his head and he was unable to speak, but was breathing. Newton Fire arrived and attempted to gain access to the vehicle via several manual tools with no success and instead deployed the Jaws of Life which was successful. Newton Fire also made entry into the rear passenger compartment because we viewed a child's car seat and checked with negative results for any child in the vehicle. Once the operator was cut out of his seatbelt and safely out of MV1 and on board Newton Medic 1, I entered the ambulance where he was identified as Mark Topham. It should be noted that Topham was having difficulty answering basic questions such as his name, date of birth, today's date, Etc. At one point when he was asked what today's date was, he gave his birth date of February 21, 1983 and it took him several attempts to say his last name, I had to run him over the air with a partial name to ascertain who he is. Also of note, in my presence and that of Sgt. Devine, Topham informed the medics that he had been consuming alcoholic beverages prior to the crash. Upon viewing the scene, I noted that the roadway was lit and I did not observe any skid marks on the dry, ice-free payment, indicating that there was unlikely any braking or swerving to avoid the collision. I spoke to the reporting party, Lindsay Giardina, she stated that she was traveling on Dedham St. when she heard what she believed to be a motor vehicle crash. When she came upon said crash, she attempted to assist the operator, as she was an off duty firefighter. She was unable to gain access to the inside of the vehicle, but communicated to the operator in the same manor as I did. She stated that she was able to smell the odor of an alcoholic beverage emanating from the operator. Topham was transported to Beth Israel of Boston, where he was assigned patient record number 3109921. I attempted to speak with him at the hospital, but was unable to do so because of his current state; I noted several injuries on Topham, such as a bloody lip, a wound to his lower left side and a wound to his left leg. Due to Topham's statements onboard of Medic 1 and Lindsay Giardina's observations, I will be charging Topham with one count of violating MGL Ch.90 S. 24 K OUI Liquor 2nd Offence, one count of violating MGL Ch.90 S. 24 E Negligent Operation of a Motor Vehicle and one count of violating MGL Ch. 89 S. 4A Marked Lanes Violation via MA Criminal Citations which I placed in the mail. Additionally, I will be seeking a subpoena for Topham's blood work from the hospital. Note: The crash occurred on the Newton/Boston line, Newton dispatch's GPS indicated that the crash was in Newton. I took several pictures of the scene and have forwarded them to IT to be attached to this report. I completed and filled an NPD Motor Vehicle Tow Inventory Form and Officer Boudrot stood by for the tow.

On 3/1/19 while working N493, myself and N492 Officer Selig were sent to the area of 1354 Washington St for a motor vehicle accident with reported injuries. On arrival, I spoke to Matthew Digregorio DOB7/2/74 ( hereby referred to as operator #1) who was standing on the sidewalk next to MA reg 134TR9 (vehicle #1 ). Vehicle #1 had significant damage to the front and right quarter panels. It was straddled between the travel lanes and did not have the keys in the ignition. It was immediately apparent that operator #1 suffered an injury to his head, as there was a moderate amount of blood and small pieces of glass stuck in his forehead area. I observed operator #1 to be unsteady on his feet, have bloodshot eyes and slurred speech. I could detect a moderate odor of alchoholic beverage coming from operator #1 as I was approaching him, which was apparent from about an arms length away (it should be noted that medics were already en route to the scene at this time). Prior to the medics arrival, I asked operator #1 for a brief synopsis of what transpired. Operator #1 stated that he had just left Paddy's Pub and was driving westbound on Hunter St (which is approximately 1/8 mile from our location) when he collided head on with vehicle #2. Vehicle #2 was not immediately visible upon police arrival, however was pointed out as being located at the intersection of Hunter and Putnam St. I asked operator #1 where he was heading, as Hunter St is a back road not directly in the vicinity of Paddy's. Operator #1 could not answer the question and also remained silent when I asked him how much alcohol he had consumed this evening. At this time the medics arrived on scene. I asked operator #1 for the car keys to reliquish to the tow truck operator, which is standard operating practice. Operator #1 reached into his pocket and then provided me with the keys. Due to concern over operator #1's obvious head trauma, he was then transported to NWH to be evaluated while the investigation continued on scene. Two other parties approached myself and Officer Selig. One was the operator of vehicle #2, MA reg 1RD999. Operator #2 is identified as Luke Ferdinands DOB 5/11/80. The other party who approached is identified as Douglas Dasilva-Pimenta DOB 6/2/86 (Dasilva-Pimenta will now be referred to as operator #3). Operator #2 states that he was taking a right onto Hunter St from Putnam St, when he observed vehicle #1 on the wrong side of the road approaching him head on. Operator #2 states vehicle #1 then collided head on with him, took a right onto Putnam St, and continued onto the Washington St overpass near 1354 Washington St. Operator #3 states he was inside of #5 Hunter St when he heard a loud crash coming from outside. He looked out the window and saw vehicle #1 traveling at a high rate of speed on Hunter St towards Putnam St. Operator #3 observed damage to the right side of his parked vehicle (now referred to as vehicle #3) MA reg 7CAK80. He immediately got in his vehicle and proceeded to follow vehicle #1, which then crashed into vehicle #2 head on. Operator #1 did not stop until it was obvious that operator #3 was following close behind. This ultimately landed vehicle #1 at it's final location near 1354 Washington St. Vehicle #1 and vehicle #2 were not driveable and were towed to Tody's. It should be noted that there was a passenger in vehicle #2 at the time of the collision. Airbags were deployed and medics checked out both operator and passenger. Both declined transport. The passenger is identified as Elizabeth Ferdinands DOB 5/22/53.Ferdinands' version of events coincide with that of operator #2. Upon speaking with Officer Selig it was learned that operator #1 dropped his cell phone several times as he was holding it on scene. Both Officer Selig and operators #2 and #3 observed this. Operators #2 and #3 both expressed concern over the obvious odor of alcohol eminating from operator #1, and the fact that he was unsteady on his feet. I responded to NWH to follow up with operator #1 however he did not wish to speak further. He did express concern over the well being of all parties involved. Operator #1 was issued in hand the following citations: T1268426 - 90/24 OUI Alcohol, 90/24 Leaving the Scene (property damage, vehicle #2), and 90/24 Leaving the Scene (property damage, vehicle #3). T1268425 - 89/4a Marked Lanes violation. T1269216 - 90/24 Negligent Operation of a M/V (mailed after consultation with superiors) All streets mentioned are public ways in the City of Newton. 1354 Washington St will be the address used on this report and subsequent citations due to multiple locations being involved. Citations and paperwork have been placed in the prosecutor's mailbox. An immediate threat form has been filled out and faxed to Mass DOT. *Attention Police Prosecutor for blood work subpoena, if applicable*

On 03/12/2019, I responded to the area of 279 Chestnut St for two car motor vehicle accident. On arrival, I spoke with the operator of MV1 (MA Reg: 824WA6) who stated that he was driving northbound on Chestnut St when he was rear ended by MV2. I spoke with the operator of MV2 (MA Reg: 4DN621) who stated that she was travelling northbound on Chestnut St behind MV1. MV2 stated that they were in stop and go traffic and misjudged the distance and rear ended MV1. MV1 sustained moderate damage to its rear end. MV2 sustained moderate damage to the front end and had airbags deployed. All parties were evaluated by medics and signed patient refusals. MV2 was towed from the scene by Tody's Towing.

On Monday March 25th, 2019 I responded to a hit and run motor vehicle accident at 115 Florence St near Louise Rd. Vehicle # 1 (Mass Reg #5TFK30 Honda Pilot operated by Sabryna Reyes) was traveling east on Florence St headed toward Route 9. According to Sabryna, Vehicle # 2 was taking a left hand turn onto Florence St from Louise Rd. Vehicle # 2 made contact with the left side of the vehicle of vehicle # 1. Vehicle # 2 drove away without staying on scene. Sabryna was unable to get a license plate of the vehicle driving away. There were no witnesses to the accident and no video cameras in the area. Sabryna described vehicle # 2 as a black honda CRV with white male passenger operator approximately in his 30s. Their were no injuries and Sabryna signed a patient refusal. Newton Fire responded because of the airbag deployment. Vehicle # 1 suffered minor damage to the left front side of the vehicle. Vehicle # 1 was towed by AAA Towing.

On 04/08/2019, at 1620 hrs, I responded to the area of 2040 Commonwealth Ave for a three car MVA with injuries. Upon arrival, I observed all three vehicles facing westbound on Commonwealth Ave. MV1 had heavy front end damage and airbag deployment. MV2 also had heavy front end damage and airbag deployment. MV3 had minor rear end damage. The operator of MV1 was brought to Newton Wellesley Hospital by Cataldo medics. The operator of MV2 stated she was on Pamela Ct attempting to take a left onto Commonwealth Ave. There was heavy traffic headed westbound and a vehicle stopped to let MV2 go. As MV2 was taking a left onto Commonwealth Ave, MV1 was traveling westbound in the eastbound lane around traffic and struck MV2. MV2 then in turn struck MV3s rear end. The operator of MV3 stated that he wasn't sure what occurred because the accident was behind him. The operator of MV1 stated she was traveling westbound on Commonwealth Ave and was planning on taking a left into the Star Market parking lot which was approximately 250 feet down the street. She stated she thought there were two lanes in the westbound direction at that point in the roadway. MV2 then pulled out in front of her and they collided. Two witnesses who were traveling westbound in traffic a few vehicles behind MV3 both stated that MV1 drove erratically into the oncoming lane and around traffic heading westbound. One witness stated "the car was almost completely in the opposite lane" and the other witness agreed. A moment later they stated they heard the crash but did not see it. They also stated that she almost crashed into a car turning eastbound out of the Star Market exit prior to the accident. MV1 and MV2 were towed from the scene by Tody's. The operator of MV2 was brought to Newton Wellesley Hospital by her mother. The operator of MV1 was cited for C90 S23 operating after revoked license, C90 S23 operating after revoked registration, C90 S24 operating to endanger, and C89 S4a marked lanes violation. All aforementioned streets are public ways maintained by the City of Newton.

I responded to the intersection of Crescent Ave.@Beacon st for a two car MVA at approx. 1343 hrs 04/25/2019. V1; MA reg 8738ES, 2010 toyt/Camry color gray operated by Kuller, Robert A. MA OLN S40421474. V1 operator was attempting to turn left onto Beacon St from Crescent Ave. There was on coming traffic on Beacon St. V1 operator who had been trying to make the turn was sticking out onto Beacon st. V1 operator put his vehicle in reverse and proceeded to back up for the oncoming traffic. V1 collided into V2 which was directly behind when he backed up. V2; MA Reg 6PA747, 2017 Jeep/Util. color white operated by Mesen, Alvarado MA OLN S31551122. V2 Operate's account of the collision was consistent with V1 operator without dispute. There was minor damage to both vehicles. Neither operator was injured as a result of the collision. No airbag deployment. Neither vehicle required towing service.

MV1 was traveling east on Madison Ave from Walnut St approaching #80. There were 2 landscape vehicles parked on left side of the street. The operator, Sarah Ruth Loper-Sengupta, stated that she never saw the pedestrian that was in the roadway (N) due to solar glare. Loper-Sengupta stated she heard a thump coming from the passenger side of her vehicle. Loper-Sengupta stated she looked over to her right and saw a trash barrel in the air. Loper-Sengupta stated she then saw a male party lying on the roadway on her passenger side. Immediately after striking the pedestrian, Loper-Sengupta, stopped to check on him. There was a golden doodle dog in the back seat of the MV at the time of the accident, unrestrained. There were no obstructions hanging from the mirror or windshield area. The front drivers visor was down. There was no airbag deployment. I observed significat damage to the front passenger side windshield area. There was also damage to the front passenger side hood area. See Sgt. Lee update for victim statement Numerous pictures were taken by Ofc. Gaudet of the MV and scene. They were TOT IT. The MV was towed by Tody's due to its shattered windshield. No citation will be issued to Loper-Sengupta. The solar glare caused her to not able to see the pedestrian. The Sgt. and I dont believe the pedestrian looked before crossing with the barell on his left shoulder because he would have been able to see the MV coming as this is straight away section of Madison Ave. It should be noted, that when I arrived on scene I immediately noticed the seolar glare as I walked east on Madison Ave.

Mv#1 was stopped at the center intersection red light on Grant Ave S/B at Commonwealth Ave. (intersection has two lights, other light prior to carriage rd). Mv#2 was travelling on Grant Ave S/B and rear ended #1. #1 operator stated, after the crash, #2 operator told him the brakes failed. #2 operator told him he would back up, pull over and get out of the way so information could be exchanged. At that time, #2 drove away on Commonwealth Ave W/B. Witness stated he observed the crash while he was at the red light from the opposite direction on Grant Ave N/B. Witness observed #2 rear end #1 and observed #2 drive away. Witness observed and wrote down license plate of #2, MA reg 8SVH20. #1 sustained moderate damage to the driver's side rear that did not require a tow. #1 stated he could feel possible knee and side (oblique) pain. Medics arrived for an evaluation. #1 signed a patient refusal. #2 operator (no injury) was later located on Newtonville Ave (incident report #19017194). #2 sustained heavy front end damage with airbag deployment. Mv#2 was towed from 148 Sumner St by Tody's. #2 issued citation for 90/24 leaving the scene of property damage and 90/10 unlicensed operation.

MV1, operator Frank Distefano (MA Reg:55EL18; a gray 2002 Toyota Highlander) was traveling NORTH on Waltham St when believes he may have fallen asleep and collided with a Eversource utility pole 39/2 along with a city green bin (06 1021851). There was heavy damage to MV1, the utility pole and the bin. There was airbag deployment but Distefano signed a refusal when medics arrived on scene. Distefano was steady on his feet, had no odor of an alcoholic beverage emanating from his person and did not exhibit red or glassy eyes. To be certain, I asked Distefano if he would consent to a field sobriety test to which he said yes. Distefano was able to perform the tests and I made the determination that Distefano was not operating under the influence of alcohol. Distefano said that he suffers from sleep apnea and states that he gets tired during the day due to his lack of sleep and prescribed medications which often makes him fall asleep unexpectedly. MV1 was towed from the scene byTody's Towing. Frank Distefano was given in hand, MA Uniform Citation T1441775 for 89/9a Marked Lanes and 90/9 Expired Registration. Due to Distefano's inability to stay awake while operating a motor vehicle, an immediate threat was filed with the RMV. Eversource arrived on scene to remedy the utility pole for public safety.

Emery Mays states that he took his fathers vehicle Ma. Reg 8BL949, this morning. Emery states that he got lost and ended up in the parking lot at 333 Nahanton St which is only accessible by the Jewish Community Center's access road and Nahanton Street (public way in the City of Newton). Emery then proceeded to do "doughnuts" in the muddied lot. Emery then lost control of the vehicle and struck a parked storage container causing minor damage. Both airbags were deployed. Emery does not have a license and was transported to NWH (as he is a juvenile and a parent could not sign a refusal in a reasonable time period ). He attends the Fusion Academy in Newton. I spoke with Emery's father Randolph Mays. I told Randolph of the accident. Randolph stated that he did not give Emery permission to take his vehicle. Randolph stated that he would meet Emery at NWH. I told Randolph that Emery was going to be charged wit Ch.90 Sec.10 unlicensed operation of a m/v, citation # T1272114 (in the mail due to his being at the hospital). I also advised that he could take out his own charges against Emery for using without authority if he chooses to do so. Todys towed the vehicle. JCC staff were notified of the incident.

On 05/15/2019, while assigned to N494, I, Officer Conary, responded to a MVA at 109 Adam Street, a public way in the City of Newton. Upon arrival, I observed a utility pole broken in half, wires hanging in the street, and also a transformer on the ground. Fire and medics responded. Other police units were able to block the street from traffic. I spoke to witness, HARRIS, who stated that she was traveling Southbound on Adam Street when she saw MV1 in the wrong lane traveling toward her at a high rate of speed. HARRIS stated that MV1 then turned into the utility pole located outside of 109. MV1 took out the pole, side swiped MV2 and then hit MV3 head on. Operator of MV1 stated that he was traveling Northbound on Adam Street when he sneezed. At this point, he said his eyes were off the road and the next thing he could remember was being in the parking lot of 109 Adam Street after the accident. There was significant damage to the front of MV1 with airbag deployment. Operator of MV1 was checked out by the medics for a head injury and signed a refusal. Operator of MV1 was asked if he had consumed any alcohol or taken any medication today and he declined. Officer Spinney (N491) asked if he was willing to do a Field Sobriety Test, and he stated yes. Operator of MV1 did not show any signs of intoxication. Operator of MV2 stated that he had parked his car outside of 109 Adam Street and was sitting in the driver seat when he was hit by MV1 and also the utility pole. Operator of MV2 was able to exit his car. He was evaluated by the medics and signed a refusal. MV2 has significant damage to the right side and windshield. MV3 was parked in the lot of 109 Adam Street facing Southbound. MV1 hit MV3 head on. There was significant damage to MV3 front end. The electric meter attached to 109 Adam Street was ripped off the house. There are no signs of damage to the house to report at this time. MV1 was towed by Tody's. Eversource arrived on scene. Operator of MV1 was issued a citation for marked lanes violation. Pictures were taken of the scene and will be submitted to IT.

On Thursday, May 16th 2019, at approximately 6:40am, I, Officer Brooks, responded to 930 Watertown street for a report of a head on collision. Upon my arrival I spoke with the operator of MV1(MA REG RT91EL) who stated she was traveling westbound on Watertown street and "blacked out" and could not recall how the accident happened. I asked what she meant about blacking out, and she stated she thought she had a panic attack and may have passed out. The operator of MV1 stated she takes Lorazepam for panic attacks, but has never had one like this. MV1 had heavy front end damage and airbag deployment, specifically to the driver's side. The operator was uninjured. I then spoke with the operator of MV2(RI COM 81082) who stated he was traveling eastbound on Watertown street, and observed MV1 crossing over the center line. The operator of MV2 stated he tried to get out of the way but MV1 continued into his lane and struck him head on. MV2 had damage to the drivers side front end. The operator stated that he was suffering from shoulder pain to his right shoulder. The operator of MV2 was transported to Newton Welleseley by Cataldo. Both MV1 and MV2 were towed by Tody's due to the damage. As a result of the collision, MV2 rolled into parked a vehicle MV3(MA REG 315PW6). There was minor damage to the rear end of MV3. The owner was notified and provided the crash information. The operator of MV1 was mailed MA Uniform citation T1442362 for 89/4a Marked Lanes violation, and additionally based on her statement of passing out at the wheel I filed an immediate threat with the registry. The operator of MV2 was mailed MA Uniform Citation T1442363 for 90/9 Expired registration.

I spoke with the operator of MV1 who stated he was traveling southbound on Lexington street, and had reached over to his passenger side to grab a Burger King bag. When he reached for the bag he drifted to the right and struck MV2. MV1 had heavy damage to the passenger side and airbag deployment. The operator was checked out by Cataldo and signed a patient refusal. MV1 was towed by Tody's. The operator was issued a citation for an expired registration. I spoke with the owner of MV2 who was inside of 163 Lexington street at the time of the accident. His vehicle had been legally parked when struck. MV2 sustained major damage to the driver's side. MV2 was towed by Tody's. Both operators were provided the report number.

VEHICLE #2 HAD EXTENSIVE FRONT END DAMAGE AND MULTIPLE AIR BAG DEPLOYMENT AND WAS ALSO TOWED BY TODYS. OPERATOR OF VEHICLE #1 WAS TRANSPORTED TO NEWTON WELLESLEY HOSPITAL FOR POSSIBLE WRIST INJURY. PASSENGER WAS ALSO TRANSPORTED TO CHECK FOR POSSIBLE INJURIES. OPERATOR AND PASSENGER OF VEHICLE #2 WERE EXAMINED BY MEDICS AND BOTH REFUSED MEDICAL TREATMENT. At approximately 0010HRs on Thursday, June 13, 2019, I was dispatched to 141 Dedham St. for a report of a two car crash. Upon arrival, I observed MV1 (NH Pass:3266403) to be parked on the side of the roadway in front of the aforementioned address and it appeared to have been rear ended by MV2 (MA Pass: 513WA3). Being that MV2's airbags were deployed, I requested Newton Fire to evaluate the operator of MV2, who was ultimately medically cleared and signed a patient refusal. Upon speaking to the operator of MV2, he stated that he was traveling Northbound on Dedham St. and feared that he was coming too close to the solid yellow divider lines and over steered to the right, accidently hitting MV1, which was parked. The owner of MV1 was on scene and stated that he was inside and came out to find the crash. I noted damage to the driver's side quarter panel of MV1 and damage to the front end/hood area of MV2. Todys serviced arrived and took custody of MV2. I completed/filled an NPD Towed Motor Vehicle Form. I cleared without incident.

I responded to the intersection of Wales St.@Washington St. for a report of an MVA. I arrived on location and observed MA Reg. 7KF936; 2018 Niss/Versa color gray parked on the northbound side of Wales St. approx. 100 yds south of Washington St. The vehicle (V1) had heavy front end damage. V1 operator was identified as Brand, Michael MA OLN#S98661403. He stated the operator of the other vehicle involved did not speak English (Spanish speaking). The female operator did exchange info with V1 operator then left prior to my arrival. V2 operator is identified as Delgado-Soto, Luz MA OLN#S83973830. V2 MA Reg. 3RH279; 2010 Jeep/Liberty color gray. V1 operator stated he rear ended V2 slowing/stopped in the line of traffic at the traffic light. He stated V2 "really didn't have any visible damage." He stated he nor V2 operator were injured as a result of the collision. There was no airbag deployment in V1. V2 Operator was contacted via telephone by Newton PD Officer Juan Garcia (Spanish speaking officer) and spoke with her. Her account of the MVA was consistent with that of the V1 Operator. She confirmed that she was not injured. Both parties have been advised. V1 required towing service from the crash scene. Tody's Towing Inc. (driver Dave) responded and towed the vehicle.

OPMV#1 Was traveling Southbound on Chestnut St when she saw an object in the middle of the road and swerved into the Northbound lane of Chestnut St. OPMV#1 Drove up over the curb and onto the sidewalk colliding with a stone wall at 542 Chestnut St and a Utility pole. The vehicle then traveled Southbound on the sidewalk and stopped at the intersection of Chestnut St and Roslyn Rd. I observed major damage to front of the vehicle along with left and right side damage. Airbags were deployed on all sides. OPMV#1 Was evaluated by EMS and she signed a medical refusal. The operator of MV#1 (International Driving Permit #1318-19-126145 with an active Korean passport expiring on 11/11/2028 with an RMV OLN of SA1730649) stated she was travelling southbound on Lowell Ave at the intersection of Austin St when she was struck by MV#2. MV#1 sustained heavy front end damages. There were no reported injuries to the operator of MV#1. The operator of MV#2 stated she was travelling westbound on Austin St and stopped for the stop sign at the intersection of Lowell Ave. The operator of MV#2 stated she then proceeded through the stop sign and made contact with MV#1. MV#2 sustained heavy damages to its entire right side with front passenger side curtain airbags deployed. There were no reported injuries to the operator of MV#2 or the front passenger. Based on the statements made to me, I issued in hand MA uniform citation #T1268346 to the operator of MV#2 for violation of c89 s9 failure to comply stop sign. Tody's Towing took possession of MV#1 and AAA took possession of MV#2. The operator of MV#2 provided me with a witness's information (Atenia at 617 981-0165). I attempted to contact the witness with negative results and no voicemail.

On Tuesday, July 23rd 2019, at approximately 5:24pm, I, Officer Brooks, responded to 444 Lowell avenue for a 2 car MVA. Upon my arrival I observed MV1(NH REG 4120041) with heavy front end damage. The vehicle was located in the driveway of 444 Lowell avenue, with the front end sticking out into the roadway. I spoke with the operator of MV1 who stated he was trying to exit the driveway and turn left onto Lowell avenue southbound, when he was struck by MV2(MA REG 3AP243) which had been traveling northbound on Lowell avenue. According to the operator of MV1, MV2 had been traveling at a high rate of speed and tried to swerve around him. There was airbag deployment in MV1 however the operator signed a patient refusal with Cataldo. MV1 was towed by Tody's. I then spoke with the operator of MV2 who stated she was traveling northbound on Lowell avenue and saw MV1 exiting the driveway. She assumed he was going to stop pulling out of the driveway, and when he did not she stated she attempted to swerve around him. MV1 made contact with the passenger side rear wheel well of MV2 causing MV2 to spin out. The operator was not injured and arranged for a private tow due to the damage in the wheel well area.

The operator of veh #2 stated she was pulling out of Tower Rd attempting to go left (Northbound) on Needham St. and didn't see veh #1 passing as she turned. Minor damage to veh #2 as the operator was cited in hand Mass. #T1271259 for N.C.O. Failure to Use Care in Turning. On 08/02/19 I responded to single vehicle crash across the street from #33 Dedham St. The operator of veh #1 stated he was driving due North on Dedham St. when his car veered into the city tree. There was airbag deployment and heavy damage to veh #1. Operator of veh #1 had some cuts and scrapes to legs but was checked out by the Medics and signed a patient refusal. The vehicle was towed by Tody's towing and the operator was issued Mass Citation #T1445105 in hand for CH89/ Sec4a Marked Lanes violation (traveling outside of breakdown lane) and Ch 90/ Sec 18 Speeding (based upon the significant amount of damage to the vehicle and how far the debris was found from the crash site) . The operator was picked up on scene by his daughter. Traffic Officer McSweeney arrived and took pictures of the damage to the city tree.

He was evaluated by Newton Paramedics and refused medical attention. The operator of MV#3 stated he was travelling northbound on Centre St when he looked to his right at a dump truck and took his vision off the street for a second when he struck MV#2 which in turn pushed MV#2 into MV#1. MV#3 sustained heavy front end damages and had both front airbags deployed. The operator of MV#3 stated both of his thumbs were hurting from the airbag deployment. He was evaluated by Newton Paramedics and refused medical attention. The operator of MV#2 does not have a driver's license and only provided a Brazilian passport. He was issued in hand MA uniform citation #T1268350 for violation of c90 s10 unlicensed operation. A licensed operator (MA license #S48386590) took possession of MV#2 and transported the operator of MV#2 back to his home. Based on the statements made to me I issued in hand to the operator of MV#3 MA uniform citation #T1268351 for violation of C.O 19-71 following too closely. MV#3 was towed by AAA towing and the operator of MV#3 was transported home by his parents.

The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave making a left turn onto Morton St when he was struck by MV#2. The operator of MV#1 stated he was unsure of where MV#2 came from. There was a strong solar glare facing westbound on Commonwealth Ave during this accident. MV#1 sustained heavy driver's side front end damages. Both front airbags were deployed. The operator of MV#1 stated he was not injured. The front passenger of MV#1 was not injured and had already left the scene to attend school. The driver's side rear passenger sustained a bleeding nose from this accident. She was unsure what caused her injuries because it happened too quickly. She was evluated by Newton Paramedics and signed a patient refusal of treatment. The initial point of impact for both vehicles was in the middle intersection of Commonwealth Ave at Morton St. There were broken vehicle parts and tire skid marks starting from the point of impact and onto the berm on Commonwealth Ave on the carriage lane side. MV#2's final resting spot was directly accross from 808 Commonwealth Ave. MV#2 sustained heavy front end damages to its driver's side wheel (Broken axle) and flat front right side tire. MV#2's driver's side airbag was deployed. The operator of MV#2 was transported by Newton Paramedics to the Boston Medical Center. I spoke to the operator of MV#2 via telephone and she stated she was travelling westbound on Commonwealth Ave when she was struck by MV#1 and caused her swerve onto the carriage lane berm. The operator of MV#1 is 6.5 months into her pregnancy and sustained neck, back, bruises and left hand injuries. She was transported to Boston Medical Center and was later discharged the same day. She was provided with this accident report number and was advised to contact her insurance provider. I spoke to the witness who was at the intersection of Cedar St and Commonwealth Ave. The witness stated MV#1 was travelling eastbound on Commonwealth Ave and had stopped, attempting to make a left turn onto Morton St. The witness stated she then observed MV#2 travelling westbound on Commonwealth Ave and struck MV#1. The witness stated she believed MV#2 was travelling at a high rate of speed because it continued to travel onto the berm after striking MV#1. I took digital photographs of the damaged berm on Commonwealth Ave. Based on the statements made to me, I issued in hand to the operator of MV#1 citation #T1268357 for violation of c89 s8 failure to yield when turning left. Both vehicles were towed by "Tody's Towing".

On October 12, 2019 at approximately 2347 hours, I responded along with multiple NPD units to a report of a MV into a pole at the intersection of Washington St and Commonwealth Ave. Upon arrival, I met with Officer Boyle who directed me to a 2007 Jeep Wrangler(MA Reg:7KK957) that was popped up onto the curb and through a traffic light pole in the intersection on the Westbound side of commonwealth Ave. I was then directed over to the vehicles operator, Sebastian Pike. Sebastian was outside of the vehicle walking around when I had arrived. Sebastian was directed to sit down for now due to airbag deployment from the accident. Sebastian was not complaining of any pain and did not need any medical attention. I then asked Sebastian what happened tonight. Sebastian stated he had just left a friends house and was driving the Jeep Westbound on Commonwealth Ave. He was driving through the intersection with Washington St when he became distracted and looked away from the road. At this point, his Jeep had jumped the curb and crashed into the traffic light pole. The traffic light pole was knocked down and blocked the road Westbound on Commonwealth Ave. Cruisers were used to block the road until Daegle Electric had arrived on scene to clear away the debris. The Jeep was towed by Tody's and a towed motor vehicle form was filed. Due to Sebastian's failure to pay attention, Sebastian Pike was issued Massachusetts Uniform Citation #T2079375 for 90/24 Negligent Operation and 89/4a Marked Lanes Violation. A copy of the report was attached to the citation and submitted to the police prosecutors box as a Criminal Application. Sebastian's Father, Jeffrey Pike, had arrived on scene and drove his son home for the night. All aforementioned streets and Avenue's are public ways in the Commonwealth of Massachusetts.

On Wednesday, October 16, 2019, I, Patrolman Hatfield, was assigned to marked cruiser N 495 to Division 5 for the 1530 to 2330 hour shift. At approximately 1742 hours, FTO Guarino and I were dispatched to Commonwealth Ave at Cedar St for a motor vehicle accident with minor injuries. FTO Guarino and I arrived on scene and noticed two MVs with damage. I spoke with the RO/operator of MV2, Janet Haines (Janet). Janet stated she was not suffering from injuries. Janet said she was operating southbound on Cedar St crossing Commonwealth Ave when she had a collision with MV2. Janet stated prior to crossing Commonwealth Ave, an unknown MV stopped at the intersection to allow her to cross. Janet then stated she proceeded accross when she crashed into MV1, who drove around the stopped MV. Janet said her brakes did not work and she continued to drive forward and ended on top of the curb of Commonwealth Ave facing eastbound. MV2 suffered damage to the whole front bumper and wheel wells. FTO Guarino spoke with the operator of MV1, Madeleine Richard (Madeleine). Be advised both front and side driver airbags were deployed, and the front driver's side door was unable to be opened. I then spoke with Madeleine. Madeleine stated she was operating eastbound on Commonwealth Ave before being involved in the collision. MV1 was struck on the driver's side by MV2, and ended facing westbound in the middle of Commonwealth Ave. EMTs and fire arrived on scene. Janet stated she did not need to be transported. Madeleine was transported to Newton Wellesley Hospital for an evaluation. Both MVs were towed by Todys.

Operator #1 states he was driving northbound on Lexington St operating MA reg 47C440 (vehicle #1). Once at the green light at Lexington/Rumford Ave, operator #1 attempted to take a left turn onto Rumford when MA reg 4FR581 (vehicle #2) collided with him. Vehicle #2 was driving eastbound on Rumford and operator #1 states he did not see the car until the crash occurred. Operator #2 reports he was driving eastbound on Rumford and was attempting to take a right turn onto Lexington St at the intersection. Operator #2 acknowledges that he had a red light but states he stopped prior to turning (right on red is allowed). It should be noted that upon my arrival on scene, vehicle #2 was located partially in the intersection with airbags deployed and I am not able to conclude that vehicle #2 was in fact turning right. The point of impact does appear to be the left front area. Operator #2 states he doesn't know what happened, and that vehicle #1 may have oversteered the left turn. The damage to both vehicles is extensive and mostly contained to the front end/undercarriage areas. Both vehicles were towed by Tody's and all parties signed patient refusals from medics on scene.

At approximately 2250HRs on Friday, December 13, 2019, I was dispatched to the area of 275 Washington St. for a report of a hit and run crash; Newton Officers and a State Police Officer canvassed the area with negative results. Upon speaking with the operator of MV1 (MA Pass:1CTB36), he stated that he was traveling Westbound on Washington St., when he observed a black Honda mini van or SUV, MV2, approach him in his passenger's side mirror and then attempt to enter his lane of travel, striking the rear of MV1. Upon speaking with the two witnesses who were traveling behind MV1 and MV2, they concurred with the operator of MV1's account. The witnesses described MV2 as a black Honda or Toyota mini van or SUV. I observed damage to the right rear quarter panel, trunk area and right rear tire of MV1. Todys Services arrived and took custody of MV1. I filled an NPD Towed Motor Vehicle Form. **Update as of 1800HRs on 12/15/2019** I was made aware that the operator of MV2 in this incident, now identified as MA Pass: 8HD155, submitted a Commonwealth of Massachusetts Motor Vehicle Crash Operator Report on 12/14/2019. In said report, the operator admits to striking MV1 and stated that he pulled over on Peabody St., where he remained for approximately 15 minutes awaiting MV1. When MV1 did not arrive, the operator of MV2 attempted to drive away, but realized the damage was too severe and returned to the same spot on Peabody St. I viewed MV2 at Clay Auto Body and observed damage to its left fender/wheel well area, as well as a deployed passenger's side airbag. Upon speaking with the operator of MV2, he reiterated the same information he stated in the above summary of his Commonwealth of Massachusetts Motor Vehicle Crash Operator Report. In the presence of his parents, I advised him on the proper steps to take when involved in an accident, such as the proper exchange of information and the potential ramifications of not doing so. Due to the operator of MV2 being 17 years old, being issued his driver's license 9 months ago and his evident remorse/confusion surrounding the incident, I will not be seeking any charges. I updated the operator of MV1 and provided both parties with an NPD Crash Data Exchange Form.

On Sunday, December 15th 2019, at approximately 3:35am, Officer Donovan and I, Officer Brooks, responded to Washington street at Saint James street, for a report of a single car MVA. Upon our arrival we observed MV1 (MA REG 1GHG66) facing westbound on the sidewalk of Saint James street. The vehicle had heavy damage to the front drivers side door, broken axles, and all of the vehicles airbags had deployed. The operator of the vehicle was sitting outside of the vehicle speaking with MA State Trooper Newman who was on scene prior to my arrival, and had witnessed the accident. Trooper Newman stated MV1 was traveling westbound on Washington street, drove over a traffic island on Saint James street, hit a light post on the sidewalk, and spun sideways off the fence that separates Saint James street from the MA Turnpike. The vehicle then struck a tree and came to rest facing westbound on Saint James street. The vehicle was only occupied by the operator. Medics were called to evaluate the operator and a patient refusal was signed. After being cleared from the medics, Officer Donovan initiated an investigation into the circumstances of the accident, and subsequently placed the operator under arrest for operating a MV under the influence of alcohol. Tody's towing was notified to remove the vehicle due to its inoperability on the sidewalk. Officer Helms took photos of the accident scene which were submitted to the IT bureau. Officer Howes completed a motor vehicle inventory form which has been filed.

The operator of MV1, Nicole Laurin, sates that she was driving southbound on Sheridan Street when she lost control of her vehicle causing her to crash into MV2 which was parked in front of 107 Sheridan Street. Both vehicles sustained major damage and airbag deployment and were towed from the scene by Tody's. Due to the high rate of speed of the crash and air bag deployment, the operator of MV1 was transported to Newton Wellesley Hospital to be evaluated. It should be noted that prior to the reported crash there was a 911 call regarding a white sedan traveling at a high rate of speed going through numerous traffic signals and signs in a densely populated neighborhood with narrow streets. After the crash several more 911 calls were received stating that a vehicle traveling at an excessive rate of speed just crashed into a parked vehicle. Based on these calls and the nature of the crash, Ms. Nicole Laurin was issued in hand Massachusetts Unifrom Citation T2013231 for 90-24, Operating to Endanger, and 90-17, Speeding over the reasonable and proper rate. Sheridan Street is a public way in the city of Newton.

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

On Tuesday, February 18th 2020 at 1908 hours, I responded to the area of 94 Vine St for a single motor vehicle accident. The vehicle (Mass Reg # 5FRT20, 2013 ELANTRA SEDAN COLOR BLUE) was being operated by William Lake. Lake was headed east on Vine St toward West Roxbury when he crashed into a City of Newton Tree. The road conditions were wet with heavy rain. Lake stated the wet conditions caused him to lose control of the vehicle. There was major front end damage to the vehicle and airbag deployment. Lake suffered a laceration to his left eye and complained of neck pain. Lake was transported to Newton Wellesley Hospital by the Medics. The vehicle was towed by Tody's. I took pictures of the accident and submitted them to the IT Bureau to be uploaded.

On 3/8/2020 at approx 1313Hrs while assigned to 497 I responded to 101 Alban Rd for a report of an elderly female with advanced dementia was missing along with husbands Lincoln Town car (see report 20012388). While on scene I was advised the elderly female, Blanche MUCCIARONE had crashed the car near 2150 Washington St. Upon arrival I found MUCCIARONE in the driver seat of Ma Reg 62344 , the front of the vehicle had major front end damage and the front driver side airbag had deployed and the vehicle was still in gear with MUCCIARONE appearing unable to understand how to place it in park. MUCCIARONE was confused and unable to communicate what had occurred. While NFD and Cataldo worked with MUCCIARONE I spoke with the operator of Ma Reg 566ET1, Patricia HESSION who stated she was stopped at the red light with her husband John HESSION in the passenger seat she was struck from behind by MUCCIARONE causing her to crash into Ma Reg 682G94 being operated by Leonid FRIEDMAN who was also stopped for the red light. MUCCIARONE was transported to NWH by Cataldo, Patricia HESSION was complaining of neck pain and elected to have her husband drive her to NWH. MUCCIARONE vehicle towed by Todys, towed Motor Vehicle Form placed in traffic box. Request for Immediate Threat Form faxed to Ma RMV Driver Control Unit 857-368-0013 .

On Tuesday, March 31st 2020, at approximately 5:11pm, I, Officer Brooks, responded to 580 Washington street for a motor vehicle accident. Upon arrival I spoke with the operator of MV1(MA REG 5TRL60) who stated he was pulling out of the parking lot of 580 Washington street, and was attempting to turn left to head westbound on Washington street. He stated he was already in the first eastbound lane of traffic when MV2( MA REG 1KL458) came at a high rate of speed eastbound on Washington street in the center lane and struck his vehicle. After striking his vehicle, MV2 hit MV3(MA REG7BG273) which was parked legally in front of 580 Washington street. The collision caused MV3 to hit MV4(MA REG 3AGN11) which was parked legally in front of MV3. MV1 had damage to the front passenger side bumper. I then spoke with the operator of MV2 who stated she was traveling eastbound on Washington street in the far right lane. She states MV1 pulled out of the parking lot of 580 Washington street and struck her vehicle causing her to loose control and hit MV3 which was parked. Her car had airbag deployment and damage to the passenger side rear wheel well as well as the front end. MV 3 had damage to the rear end and front end. MV4 had minor scratches to the rear bumper. Nobody was injured in the accident, and the operator of MV2 signed a refusal with Cataldo. Tody's towed MV2 due to the damage. A motor vehicle inventory was filled out and filed.

On 6/26/2020 at approx 1214hrs while assigned to 497 I responded to the area of Washington St at Quinobequin Rd for a report of a 2 car crash with air bag deployment. Upon arrival I observed Ma Reg 579AM3 a gray Toyt Rav 4 and Ma Reg 219w70 a red Toyota Corolla in the middle of the EB lanes of Washington St both with significant damage, front and side airbag deployment and fluids leaking. I ascertained the operators were not hurt and called for two tows and NFD for speedy dry. I spoke with the operator of the red Toyt Brendan OCONNOR who stated he was travelling EB on Washington St, had the green light, was proceeding through the intersection when he was struck by the operator of the Toyt Rav 4,Gabriella NUGENT. I spoke with NUGENT who stated she was attempting to take the left onto Quinobequin Rd from the WB lane of Washington St when she collided with OCONNOR. NUGENT acknowledged that although the light was green WB she did not have the left arrow to turn. Both cars towed by Todys, both Nugent and OCONNOR signed patient refusals with Cataldo ambulance.

On Wednesday, 7/1/2020, at 2230hrs, I was dispatched to the area of 856 Watertown St for a report of a possible car crash. Watertown St is a public way as it is owned and maintained by the City of Newton. Upon arrival, I met with operator, Henok Kiflit, who was out of the vehicle and very disorientated. I observed a gash on his head right above his eye and noticed all airbags had been deployed in his vehicle. I asked him to sit down and I radioed for an ambulance. Kiflit stated that he was driving EAST on Watertown St when he collided with a unoccupied parked car (a red 2015 Jeep Wrangler, MA Con R22519) which was parked on the street outside of 856 Watetown St. Kiflit was driving his girlfriend's car (a gray 2020 Nissan Sentra, MA Reg 1ANN57) and was on his way home in Watertown. When I asked for Kiflit's license, he told me that he only has a permit and has yet to take the test. He apologized repeatedly. Kiflit did not have any passengers in his vehicle. Kiflit said the roadway was so dark and he didn't see the parked Jeep. He claimed he was not speeding. There was minimal damage done to the drivers side rear bumper of the Jeep. The Nissan had extensive front end damage and was subsequently towed by Tody's Towing. Towed motor vehicle form filled out and filed, copy made for police prosecutor. Owner of the Jeep came out and took possession of the vehicle. Kiflit was taken to NWH for evaluation of his injuries. Kiflit's driver history shows that he had an surchargable accident recently 05/30/2020. Massachusetts Uniform Citation T2080927 was filled out and prepared to be mailed to Kiflit at his Watertown address. Henok Kiflit is being cited for: 90/10 - Operating without a License (Learner's Permit violation; Kiflit, operator, was not accompanied by any motorist)

I responded to 1641 Beacon St. for a report of an MVA/Hit &Run. I arrived on location and spoke with Alicia Ianiere MA OLN#S84312664. Alicia's vehicle was parked when the car was hit by a passing vehicle. Alicia was inside a restaurant picking up food. The vehicle was occupied by her daughter Katherine Cullen 23 Y.O., who was seated in the front passenger seat, at the time of the collision. Katherine stated she had some stiffness in her neck. EMS responded to the scene to evaluate her. There was no airbag deployment. Katherine was cleared by EMS, she was not transported. The vehicle that was hit was MA Reg. 1JXF31; 2018 Toyt/Camry sedan color brown. The vehicle was hit by an unidentified vehicle. RP Alicia Ianiere stated the vehicle struck the rear of her vehicle as it was parked in front of 1641 Beacon St. The vehicle continued Westbound failing to stop. Ianiere's Vehicle sustained damage to the rear driver's side. There were several people seated at an outside area of a nearby restaurant. Those I spoke with heard the crash but did not actually observe the collision. There was one witness identified who stated he observed the collision. witness was identified as Dylan Shuler 24 Y.O.. Mr. Shuler described the other vehicle as a "silver2 door Honda Civic. He stated the plate was either MA Reg. 6EJZ30 or 6ZJZ30. A query of the possible plates came back not matching the vehicle description. The other witnesses also described a silver 2 door Honda. Nobody else observed a plate number. Alicia Ianiere was advised to notify her Ins. company and that I would document the accident as a MVA Hit&Run.

Upon arrival I spoke with the operator of MV1(MA REG 1XKP51), who stated he was traveling southbound on Cherry streeet, and failed to stop for the red light at the intersection of Webster street. The front end of his vehicle struck MV2 (MA REG 443LP3) on the passenger side front end. MV1 had damage to the front end and was towed by Tody's. I spoke with the operator of MV2 who stated she was traveling westbound on Webster street and had the green light. As she proceeded through the intersection she was struck by MV1. Pictures were taken and submitted to IT. The airbag was deployed. Perez complained of chest and head pain. Perez was taken by Ambulance to Newton Wellesley Hospital. There was extensive damage to the front of the vehicle. Tody's Towed the vehicle. A Towed Motor Vehicle Inventory form was completed. Perez was mailed Mass Uniform Citation #T1273442 in the amount of $ 105 for M.G.L Ch. 89/9 Failure to Stop at Stop Sign.

The operator of MV1 states that he was driving eastbound on Brookline street when another motor vehicle entered his lane of traffic head on causing him to swerve to the right and hit two city trees. No other vehicle remained on scene. MV1 sustained major damage and was towed from the scene by Todys. Due to significant airbag deployment medics evaluated the operator of MV1 who signed a patient refusal. Pictures of the city trees were submitted to IT.

The operator of MV1, Mr. Tong Zhi, stated that he was traveling west bound on Needham street when he fell asleep causing him to lose control of the vehicle and crash into a Verizon utility pole 305/17. The motor vehicle sustained major front end damage and full airbag deployment. The utility pole was snapped, and was leaning as a result of the impact. Mr. Zhi stated that he had wrist and chest pain due to the crash and was transported to Newton Wellesley Hospital by Cataldo. Todys arrived on scene and towed the vehicle. Verizon was notified regarding their damaged utility pole. Photographs of the crash were taken and place in the IT Bureau's mailbox. Due to the facts that Mr. Zhi was asleep at the wheel while traveling on a major public way in the City of Newton, that he crossed the double yellow line into the on coming travel lane, and subsequently crashed into a utility pole causing heavy damage, Mr Zhi is being charged with 90-24 Operating to Endanger. A Massachusetts Uniform Citation, #T2080141, was completed back at the station for the offenses of 89-4A Marked Lanes violation and 90-24 Operating to Endanger. The violator's copy of the citation was placed in the Chiefs mailbox to be mailed to the offender while the criminal application portion was placed in the court prosecutors mailbox attached to this report.

The witness stated she was travelling southbound on Lexington St a few vehicles behind MV#1 and observed MV#1 veered towards the northbound travel lane and striking the city tree and retaining wall of 106 Lexington St. The witness then pulled over and assisted the operator of MV#1. The operator of MV#1 stated she was travelling southbound on Lexington St and was "multi tasking" inside her vehicle when she veered off into the northbound travel lane, jumped the curb onto the sidewalk and got wedged in between a city tree and the retaining wall of 106 Lexington St. The operator of MV#1 was not injured but could not exit her vehicle because the doors were wedged in between the tree and retaining wall. MV#1 sustained heavy front end damages, broken front passenger window, front passenger wheel and deployed driver's side airbag. The operator of MV#1 stated she was not injured and was evaluated by Newton Paramedics and signed a patient refusal. The operator of MV#1 was coherent and appeared to be of sound mind and health. I transported the operator of MV#1 to her home and Tody's Towing took possession of her vehicle. Traffic unit 535 took photographs of the damaged city tree.

On 9/1/20 while working N493, I responded with N497 and the Fire Dept to 40 Crescent St for a report of a car into the house. On arrival I observed MA 6CM450 in the backyard of #40 unit C. It was stationary against the large AC Compressor in the rear of the unit, and appeared to had run over several large shrubs before coming to a stop. I spoke with vehicle owner and operator Marion Cohen 8/27/46. Cohen stated that as she was parking in the rear lot of the complex, she accidentally mistook the brake for the gas pedal. She in turn ended up going over the curb and through some bushes before hitting the compressor. There was a passenger in the vehicle identified as Milton Kornfeld 3/10/42. Both occupants were checked out by medics and declined transport. The airbags did not deploy. There does not appear to be any structural damage to anything other than the compressor. NFD checked the integrity of the house, and the compressor was unplugged. I took pictures of the scene and submitted the memory card to IT. Homeowner Xiaozhi Zhao and his family were present inside at the time of the collision and are unharmed. Tody's responded and removed the vehicle from the property. It sustained heavy front end damage and was towed to their lot via flatbed. A m/v inventory form was filled out. This report number has been provided to all parties for their respective insurance companies.

She was transported by Newton Paramedics to Newton Wellesley Hospital. The operator of MV#2 had a small dog with her that was located in the front passenger seat. Newton Animal Control took possession of the dog and transport it to the veterinarian. It was reported that the dog sustained possible internal trauma. The operator of MV#3 stated he was pulling out of the driveway of Walgrens onto Walnut St and struck MV#2. The operator of MV#3 stated MV#2 was travelling at a high rate of speed. MV#3 sustained heavy front end damages. There were no reported injuries to the operator of MV#3. Tody's Towing took possession of MV#2 and MV#3. MBTA Police Officer Michael Bolotin arrived on scene and documented the accident (MBTA case #2020-5909). MBTA Officers took possession of MV#1. The City of Newton DPW's street sweeper cleaned the debris on the street.

I responded to the area of 63 Pierrepoint Rd for a report of an MVA single car crash into a tree. I responded to the area at approx. 1752 hrs 09/19/2020. I was advised by NPD Dispatch that the vehicle was unoccupied and that the vehicle has been there all day according to the anonymous reporting party.. When I arrived on location I observed MA Reg. 31542 off the roadway facing westbound directly in front of a tree. This was located directly in front of the residence of 63 Pierrepont Rd. The vehicle was unoccupied with heavy front end damage and front and side airbag deployment. The windows were left 2/3 of the way open and the cars ignition key was left on the driver's seat. The vehicle had collided into a city tree. There was debris from the vehicle at the base of the tree. The tree had visibly been hit but there was no damage to the tree by my observation. I contacted the vehicle owner Edward G. Murray Jr. He confirmed that he had knowledge of the crash. His son Edward G Murray III was the operator at the time of the crash. Murray III had a female passenger Diana Moreno 07/12/2004. I spoke with both Murray Jr. and Murray III via telephone. Murray III stated he was driving following his GPS and suddenly an animal which he believed to be a small dog suddenly ran in front of the car. Murray III swerved to avoid hitting the animal and drove off the road and collided into the tree. He stated the crash was at approx. 0950 on 09/19/2020. I also spoke with Diana Moreno (passenger), via telephone. She could not tell me what preceded the crash as she was looking at her cell phone just prior to the crash. Murray III and Moreno confirmed they were not injured as a result of the crash. Murray III contacted his father and informed him of the crash. Neither party contacted police. Murray Jr contacted AAA to respond advising the vehicle needed to be towed. Murray Jr. stated he was not further contacted by AAA and assumed that the vehicle was towed. Murray III and Moreno left the vehicle parked off the roadway as AAA was not immediately responding. I photographed the crash scene. The photo memory disk has been submitted to NPD IT bureau. The vehicle was towed by Tody's Service Inc.

On Wednesday, October 21st 2020 at 0341 hours, Myself N-499 and Ofc Crowe N498 responded to 524 Parker St for a single vehicle accident into a city tree. Upon arrival, I identified Mass Vet #VT36122, 2016 Dodge Sedan, Color Black had crashed into Verizon Pole number 340/50A. The airbags had been deployed. The operator was no longer in the vehicle. The operator was sitting down on the ground nearby . He was identified as Joshua Stomberg. Stomberg admitted he had operated the vehicle. Stomberg stated he was driving south on Parker St toward the VA Hospital for his work shift. Stomberg stated he was tired and did not feel alert. Stomberg also stated there was a heavy fog that made the road difficult to see. Stomberg's vehicle veered off the side of the road and collided with the pole causing heavy front end damage. Stomberg did not show any signs of being impaired by alcohol. An inventory of Stomberg's vehicle showed no discovery of any alcohol. It should be noted that on my way to the accident scene, I observed a very heavy fog on Parker St that made it difficult to see the road in front of me. Stomberg suffered an injury to his back and was transported to the VA Hospital by Cataldo Ambulance. Tody's towed the vehicle. Stomberg was mailed MA Uniform Citation # T2013041 for the amount of $ 105 for M.G.L Ch 89/S4A Marked Lanes Violation. Pictures were taken of the accident and submitted to Newton IT Bureau.

Upon arrival, I identified Mass Reg # 3GDM90, 2015 LEXS RX350, Color Gray had crashed into a City Tree. The airbags had not been deployed. The operator was no longer in the vehicle upon my arrival. He was identified as registered owner Brian Mcneill. Mcneill admitted he had operated the vehicle. Mcneill stated he was driving North on Parker St toward RT 9 headed home to Wellesley. Mcneill stated he was on his phone texting and lost sight of the road. Mcneill's vehicle veered off the side of the road and side swiped a City of Newton Tree causing heavy right side damage and loss of the front right tire. Mcneill's wife came to the scene to pick him up and drive him home. The set of golf clubs in the trunk of the vehicle was documented in the MV Inventory Crash Form. Mcneill suffered no injuries and signed a patient refusal with the medics. Tody's towed the vehicle. Mcneill was given MA Uniform Citation # T2013042 for the amount of $ 210 for M.G.L Ch 89/S4A Marked Lanes Violation and M.G.L Ch. 90/S13B Texting While Driving Violation. Single MVA. MV lost control on the roundabout on RT 16 Washington St heading EASTBOUND, hit an island and was laid to rest on the sidewalk of the MassPike bridge on Rt-16. DAngelo in front of NPD HQ that there was a crash and two occupants got out, a man and a woman. Lt. DAngelo was able to locate the male walking on Washington St who was involved in the crash, later identified as Jonathan Kardas. He stated that he was the passenger and his friend who was driving the vehicle but ran away. Multiple units looked for the female individual who was Sonia Walker, the owner of the MV. On 12/10/20 at approximately 13:23 myself along with other Police, Fire and Ambulance units, responded to the intersection of Chestnut St. and Elliot St. for a two vehicle crash. Upon arrival I spoke with the operator of veh#1 who stated she was driving on Elliot St. due West when she proceeded through the intersection and was struck on her passenger side by veh #2. The operators 2 young children were in back of her vehicle in child seats. Her son was behind the passenger and her daughter was behind her. Both front and side airbags were deployed in veh #1. All three people in the vehicle were taken to Newton-Wellesley Hospital to be checked out as a precaution. The operator stated she had pain in her right wrist. The operator of veh#2 stated he was traveling due South on Chestnut St. when he looked down as he approached the intersection. The operator stated he was prepared to take a right onto Elliot St. but when he looked up he had already entered the intersection and struck veh #1. The air bags went off in veh #2 as the operator had cuts on his left hand . He was checked out by the Medics and signed a patient refusal. The operator of veh #2 had left the scene after I initally spoke with him but while I was attending to the occupants of veh #1. Because of this I was unable to issue him a citation on scene. I mailed him Mass. Citation #T2080850 for Ch 89/ Sec 9 Failure to obey the Red Light. Both vehicles sustained heavy damage and were both towed by Bobby and David from Tody's towing.

MV1 stated she was attempting to manuever around vehicles to exit the street when "I stepped on the gas and not the brake" while in reverse subsequently going backwards, over a curb, across the lawn, and into the bushes of 17 St James Ter sustaining heavy damage throughout the vehicle. The homeowner of the residence reported damage to his curb, lawn, bushes and electrical conduit (Newton Fire checked the residence prior to clearing). Todys Towing responded and removed the vehicle from the lawn and leaving it on the street as the owner wanted AAA to respond. Due to airbag deployment, Fallon Ambulance responded to the scene, however the driver refused to be evaluated (also refused to sign a refusal).

On 1/2/2021 at approx 0847hrs while assigned to 497 I responded to the intersection of Washington St and Quinobequin Rd for a report of a MV Crash. Upon arrival I observed Ma Reg 35GM68 with front end damage stopped in the right eastbound lane of Washington St at the intersection of Quinobequin Rd, the operator, Silvaneide FADIL was still inside the vehicle with all front airbags deployed. FADIL denied being injured and stated she was coming through the green light from Wales St when the truck in front of her suddenly stopped causing her to crash into it. I spoke with the operator of the truck, who had managed to pull off to the side of the road, Joshua VINCENT, Ma Reg 1182BY, who stated he had just started into the intersection from Wales St after getting the green light when he observed a red Ambulance with lights flashing approaching from Quinobequin Rd, he stopped to allow the ambulance to pass when he was struck from behind by FADIL. VINCENT expanded that as he had just started into the intersection, he was going at a slow speed and did not need to stop suddenly. I was approached by a witness, Donald MCCARTHY( 617-438-1207) who stated he observed the ambulance come though and felt FADIL could not have been paying attention to not see it and stop. Both FADIL and VINCENT signed patient refusals with Fallon Ambulance, FADILS vehicle towed by Todys Towing.

Roselourdes Pierreadrien was operating vehicle #2 S/B on Chestnut St. Roselourdes states that she had a green light and was proceeding through the intersection and was struck on the right side by vehicle #1. Roselourdes vehicle rolled over and landed on its roof. Lourdes was able to free herself from the vehicle prior to my arrival. Stuart Schneller was operating vehicle #1 on Elliot St E/B. Stuart stated that he believed he had a green light when proceeding through the intersection. Both parties suffered injures and had a hard time recalling what had happened. Both parties transported to NWH and both vehicles towed by Tody's Off. Kane was on a detail nearby. Off. Kane states that she did not see the accident happen but heard it. Off. Kane then saw vehicle #2 roll onto its roof. Off. Kane then went over to the vehicle, broke the drivers side window, cut open the airbag and assisted Roselourdes from the vehicle. Off. Kane went to NWH for injuries to her hands where she was eventually discharged.
