Motor vehicle 1 (MV1) was traveling eastbound on Beacon Street, when unknown motor vehicle 2 (MV2 ) pulled out from a parking space in front of 706 Beacon Street, without signaling, and crashed into the front passengers side of MV1. MV1 pulled over to the side of the road to exchange information as MV2 fled the scene eastbound on Beacon Street. The operator of MV1 stated that MV2 appeared to be an older model sedan, possible maroon or red in color, and was operated by a white female in her 60's. MV1 sustained significant front end and passengers side damage. MV2 likely sustained significant front drivers side damage. I was unable to check the surrounding businesses for possible security footage of the crash as all of the surrounding stores were closed when I arrived on scene.

MA commercial reg M94754 (vehicle #1), a City of Newton pickup/sander truck, was located crashed over the curb in front of 1970 Beacon St. The vehicle was unoccupied, had the keys in the ignition, and was reported stolen just a few minutes prior to discovery. The reporting party of the stolen truck and uninvolved occupant is identified as City DPW worker George Sementelli. The witness/passerby who discovered the collision is also a DPW worker, identified as Donny Brandt. Pursuant to an investigation outlined in incident #19002213, the operator at the time of the crash was determined to be Michael Carter. It is believed that Carter stole the vehicle from 295 Auburn St, fled the area, and ultimately crashed in front of 1970 Beacon St. Corroborating facts outlined in the incident report put Carter as the operator. The vehicle itself was located partially on the sidewalk and partially on the road, therefore Carter is being cited with MGL 89/4A - Marked Lanes Violation (T1268423). Due to the fact it is believed that Carter fled the scene after the crash, MGL 90/24/C - Leaving the Scene of Property Damage was added as well as MGL 90/23/D Operating After a Suspended LIcense, and MGL 90/24/O - Reckless Operation of a Motor Vehicle. The crash occurred during busy weekday commuting about 0.7 miles from Angier Elementary School. The last 3 charges mentioned have been listed on a second citation (T1268422). Officer Brooks also issued T0648082 for OUI Drugs MGL 90/24/G. See the incident report for details about this charge. In total, there are (3) citations issued to Carter. The vehicle sustained heavy front end damage. It should be noted that the utility pole a couple of houses down from the crash site (Beacon/Varick Rd, pole #212) sustained fresh, minor superficial damage, consistent with the vehicle crashing into it before verring onto the sidewalk. Pictures have been taken and submitted to the IT bureau. Tody's responded and towed the vehicle to the police garage for processing due to the pending other criminal charges involved in this accident/incident. Michael Carter was subsequently placed under arrest and his citations have been placed into his property. Carter suffered a hand injury due to the crash an was transported to NWH Hospital for an evaluation after he was booked.

Motor vehicle 1 (MV1) was parked facing southeast in front of 97 Union Street, when unknown motor vehicle 2 (MV2), sideswiped MV1 and then left the area. As a result of the crash, the passengers side, side view mirror was ripped of MV1. MV2 likely sustained minor drivers side damage. The reporting party, Gianfranco Palestro, heard the collision and believes that MV2 was a dark colored SUV, but was unable to get a license plate number. I was unable to check if any of the local businesses had security cameras that may have captured the crash as they were all closed for the night.

The owner of MV#1 stated her vehicle was parked in the rear parking lot (Located on Rice St) of 1066 Centre St and was informed by 2 witnesses that stated her vehicle was struck by MV#2. I spoke to the 2 witnesses and they stated they were on the second floor of 1066 Centre St in suite #212 and had a bird's eye view of the rear parking lot on Rice St. The witnesses stated that MV#1 was parked while MV#2 was parked at an angle at a very close proximity to MV#1. The witnesses stated the operator of MV#2 was cleaning the snow off her vehicle, got into her vehicle then backed out of her parking space striking MV#1. The witnesses took a digital photograph of MV#2's registration (MA REG 763YC9). They described the operator of MV#2 as an Asian female in her 40's, 5'1" in height, operating a black colored "Buick" S.U.V type vehicle. MV#1 sustained minor damages to its front passenger door and bumper. A query of MA REG 763YC9 showed that it was registered to an Asian female to a black colored "Buick Rendezvous". The owner of MV#1 stated the owner of MV#2 also has a business in the same building at 630 Commonwealth Ave (Move2Boston Group). I spoke to the registered owner of MV#2 via telephone and she stated she was parked in the rear parking lot on Rice St but denied making contact with any vehicles. I could not observe MV#2 as it was not at the scene. At 14:40 I returned to 1066 Centre St and was able to observe MV#2. MV#2 displayed minor scrapes to its front driver's side bumper which was consistent relative to where both vehicles were parked. Based on the witnesses statements I will document this accident and provide this report number to the vehicle owners.

On Tuesday, March 5th 2019, at 7pm, I, Officer Brooks, responded to 33 Walker street for a past hit and run. On my arrival I spoke with the operator of MV1(MA REG 28DG82), who stated his vehicle was struck at 3pm this afternoon in front of 1296 Washington street. His vehicle was alledgedly struck by a bus, unknown plate or company, that was traveling eastbound on Washington street. The bus casued the front drivers side quarter panel to be ripped from the vehicle. MV1 was towed by AAA.

The operator of MV#1 stated she was travelling southbound on Walnut St at the intersection of Elm Rd when she let a vehicle out of Elm Rd to make a left turn. The operator of MV#1 stated she proceeded to move forward when a school bus directly behind that vehicle also made a left turn making contact with MV#1. The operator of MV#1 honked her horn to get the school bus's attention with negative results. MV#1 sustained minor damages to its driver's side front bumper and fog light. The damage to the school bus should be to its driver's side rear bumper. There were no reported injuries to the operator of MV#1. There were no reports of any contracted school bus operators reporting an accident. The operator of MV#1 stated there were witnesses at the accident scene but she did not get any of their information. The operator of MV#1 was advised to contact her insurance provider.

On Monday, March 11, 2019 while assigned to Traffic unit N525, I responded to the area of Watertown Street and Adams Street, Newton for a report of a motor vehicle/pedestrian accident. The accident was reported to Newton Police Dispatch by Newton Police Officer Charles Guarino. Officer Guarino identified the pedestrian involved in the accident as Newton Police Officer James Selig. Watertown Street and Adams Street are both public ways maintained by the City of Newton. After completing my investigation, the operator of MV1, Colleen Butaney (S20012566) was issued Massachusetts Uniform Citation T1272995 for Chapter 90, Section 25 (Fail to Stop for a Police Officer) and Chapter 89, Section 4A (Marked Lanes Violation). Butaney was advised and the citation was placed in her property in Locker #4 in the NPD holding area prior to her release. Officer T. McCarthy completed and submitted a criminal application for Butaney for Chapter 265, Section 15b (Assault with a Dangerous Weapon, MV). Please refer to incident report #19009204 for statements from all parties involved. Photos were taken of the area/vehicle and submitted to the IT Bureau. The vehicle involved, (MA: 531PX6) was removed from the roadway by Tody's towing. Officer Marini completed a towed motor vehicle report form. Surveillance footage of the incident was recovered from a local business and was submitted to the IT Bureau/Support Services. A property and evidence form was completed and submitted to Officer Manouk.

MV1 had just picked up a student at 56 Crescent Ave(at the intersection of Throwbridge). He made the pick up just west of the stop sign. He turned off his illuminated bus stop sign, paused and then began to pull into traffic. MV2 was stopped at the stop sign on Trowbridge st at Crescent St waiting for the bus to turn off the stop sign. When the bus turned it off, it appeared to him that the bus was not pulling into traffic. MV2 began to turn left "very slowly". As he was driving next to the van, the van pulled into traffic and the 2 MVs touched mirrors. There was no damage to either MV. A BLS ambulance was called to the scene because there were three students on board. All parents showed up and signed refusals for their children. Pictures were taken of the two mirrors that had no damage. The following parents showed up on scene to sign refusals for their children: Gareth Gorman for Sophia 617-515-1576 Lori for Harry Melchionno 617-527-7751 Malagros Falu for Nathalie Cedno The students belong to the Oak Hill Middle school and they were notified.

Narrative:
On Friday March 15th 2019 at approximately 0730 hours I responded to the intersection of Beacon Street and Herrick Road for a report of a hit and run involving a bicyclist. The weather at the time of the accident was cloudy and the road conditions were dry. Both streets are public ways in the City of Newton. Upon arrival I observed the victim identified as Glenn Butcher (MA OLN S36552335), inside the ambulance getting medical attention. Butcher was getting a bandage applied to his left leg and complained of leg, back and neck pain. Butcher appeared visibly shaken and in pain. Butcher stated he had the green traffic light and was travelling eastbound on Beacon Street crossing Centre Street and then continuing straight crossing Herrick Road. Butcher stated MV1 was travelling westbound on Beacon Street and took a left turn onto Herrick Road and collided into his bicycle. Butcher states upon impact he flipped over his bicycle and landed on the hood of MV1, before rolling off and landing on the ground. Butcher states he was wearing his helmet which was present. Butcher states his bicycle ended up partially under MV1. Butcher states the operator of MV1 pulled his vehicle to the side of the road and got out of his vehicle and spoke to several witnesses briefly. Butcher states the operator then left the scene without properly exchanging information and did not wait for the police to arrive. Butcher described the vehicle as a dark gray Honda, and described the operator as an older white male, 5'10 approximately 80 years old with white hair. Butcher was also able to provide an email address for another bicyclist who witnessed the accident. The witness left his information with Butcher prior to my arrival. Fire fighters on scene were able to get the registration number of the vehicle from the witness (MA REG 4CX148) and gave it to Officers on scene. Butcher was transported by Medic 1 to NWH with minor injuries. His bicycle had visible damage to the frame area and skid marks were present on both tires. 19 photographs of the scene and the bicycle were taken and sent to the IT dept. email. Officer Gabriel transported the bicycle to the station where it was logged into Property & Evidence for safekeeping. Dispatch was able to query the registration which came back to a gray, 2010 Honda Accord registered to a Matthew Sugarman (MA OLN S38601397), out of W. Roxbury. Dispatch contacted the Boston Police Dept. and had them send an Officer to the address to try to make contact with Sugarman. Sugarman was advised to contact the Newton Police Dept. and I was able to speak with him on the telephone. Sugarman states he was driving the vehicle today and was involved in an accident with a bicycle. Sugarman states he was travelling westbound on Beacon Street and was taking a left onto Centre Street not Herrick Road. While speaking with Sugarman he seemed confused as to his location during the accident and was adamant that he was turning left onto Centre Street and not Herrick Road. I advised Sugarman that I was on scene with the first responding Officers and the accident clearly occurred at the intersection of Beacon Street and Herrick Road based upon the final resting place of the bicycle. Sugarman went on to state that his vehicle was already turning when the bicycle struck his vehicle and he believed it was the bicyclists fault. I advised Sugarman it appears the bicyclist had the right of way since it was traveling straight and that because his vehicle was taking a left turn, he failed to yield for on coming traffic at an intersection. When asked why he did not stay at the scene of the accident Sugarman stated he got out of his vehicle and spoke to the bicyclist and he "seemed okay". Sugarman states he then left the scene while the victim was still sitting on the ground. I asked Sugarman if he had ever been involved in accident before and he replied "no." I advised Sugarman that I am currently looking at his drivers history and see nine accidents that he was involved in. Sugarman stated "he has never been in accident like this before." I then asked Sugarman what the law states when you are involved in an accident and he was not able to provide me with an answer. I advised him that the law states you must stop and exchange information before leaving the scene of an accident. Again he stated the bicyclist seemed okay and "he had somewhere to be so he left." Sugarman has an extensive drivers history including nine surchargeable accidents, reckless operation, two different charges of leaving the scene property damage and two immediate threat affidavits filed against him. I advised Sugarman that I will be mailing him a citation for Ch89 s8 Fail to yield to oncoming traffic, and Ch90 s24 Leaving the scene of personal injury. I also filed an immediate threat form with the registry. I emailed the witness using the information he provided to the Newton Fire Dept that was on scene and received a phone call back from the witness identified as Bruce Adams. See witness report for his statements. I contacted the Bank of America located at 808 Beacon Street in Newton, as they are the closest business to that intersection with video surveillance. I spoke with George Walsh (781-834-8275) who stated they do not have any working cameras on that side of the building to the bank. For this report the bicyclist is entered as a non-motorist for validation purposes. Witness Bruce Adams (MA OLN S96398108) stated that he was riding his bicycle travelling behind the victim about 100 feet away. Adams states he was watching the victim and had a clear view of him as he travelled behind him, through the green traffic light heading eastbound on Beacon Street. Adams stated he witnessed MV1 travelling westbound on Beacon Street take a sharp left turn onto Herrick Road which turns into Union Street. Adams stated that the vehicle turned left colliding into the bicycle and there was no possible way the bicyclist had time to stop to avoid the collision. Adams stated the victim flew over his handle bars and fell on top of MV1's hood and then landed on the ground. Adams stated the victim's bicycle was stuck under MV1's front bumper. Adams stated the operator of MV1 backed his vehicle up with the bicycle still attached to his vehicle bumper and pulled MV1 over to the side of the road. The operator then got out of his vehicle and walked up to the victim who was laying on the ground visibly in pain. Adams stated the operator asked the victim if he was okay to which the victim did not immediatley reply as he was visibly shaken up. Adams stated the operator then walked back to his vehicle and attempted to leave the scene and that is when Adams stood infront of MV1 to stop him from leaving the scene. Adams stated he then removed the bicycle from underneath the bumper of MV1 and placed it on the side of the road. Adams stated the operator got out of his vehicle again and walked up to the victim and asked him "what do you want to do?". Adams stated the victim did not respond and that is when the operator got back into his vehicle and drove away, leaving the scene before any emergency response arrived. Adams stated he was able to memorize MV1's registration and stayed with the victim until police, fire and medics arrived. He stated he gave the fire dept the registration and his contact information prior to leaving the scene.

Mv#1 operator was towing a disabled Mv (#2) with a dolly wheel trailer on Cummings Rd W/B. #1 turned right N/B going uphill on North St from Cummings. At that time, the safety chain broke free from the wheel attached to the trailer. #2 dislodged from the trailer and rolled down North St about 40-50ft over a small retaining wall and into the front edge of the residence at 66 Cummings Rd. Appears to be no damage to retaining wall, minimal damage to corner of house and some bushes in front of the house. No injuries. #2 was pulled away from the house with no further damage by Tody's using the ramp flat bed truck. Tody's towed #2 to their storage yard. Home owner was notified of damage.

Operator of Vehicle #1 stated that he was traveling North on Centre Street. He passed through the intersection of Cabot and Centre street where he proceeded downhill on an icy road. Operator stated that he "fishtailed" and lost control of his vehicle before leaving the roadway and coming to rest in the shrubs and trees in the front yard of 700 Centre Street. At the time of this incident there is a reported water main break at the intersection of Centre street and Sargent street with City Water Crews addressing the issue. I observed a large collection of ice in the intersection and in the northbound lane of Centre street. No injuries were reported. Tody's was notified and removed the truck from the bushes. Operator #1 drove the vehicle away. I notified the home owner, Mr. Watkins of the damage. He estimated the damage to the landscaping to be approximately $2000.

MV1 a City of Newton school bus was stopped southbound on Parker St at Rte 9 east to turn left onto the ramp. MV2 began passing the bus on the right when the bus turned left onto the ramp. The right rear corner of the bus struck the front left wheel well of MV2. I took pictures of the damage to MV2 and TOT IT. I spoke with Regina Moody of transportation and she put me in contact with the driver of a bus who was in the area at the time of the MVA. On Thursday I met with Joseph Montuna who was in the area at the time of the MVA. His bus is 362. The side of his bus says Eastern Bus Company. It does not say Newton Public Schools anywhere visible. I looked at the right rear of the bus that would have come into contact with the MV. There was not even a smudge on the bumper. I am not positive that this is the bus involved. I have attempted to contact MV2 for clarification to positively ID the correct bus. The number provided goes directly to VM which is full. It is possible that this was a non Newton school bus involved that I am unable to identify.

On Wednesday April 10th, 2019 at approximately 0755 hours I responded to the area of Centre Street and Washington Street for two car motor vehicle accident involving a Newton school bus with children on board. Centre Street and Washington Street are both public ways in the City of Newton. At the time of the accident the weather was cloudy with light rain. The road conditions were fairly dry. I spoke to the operator of MV1 identified as Julie Lym (S16060051) who stated she was traveling northbound on Washington Street onto the Centre Street bridge. MV1 stated she was attempting to cut across lanes to merge on to the Mass Pike Westbound. MV1 stated traffic was at a standstill and she saw an opening to change lanes. MV1 stated MV2 was stopped in front of her and as she attempted to change lanes her vehicle side swiped the rear drivers side corner of the school bus. The operator of MV1 reported no injuries. MV1 sustained minor damage and was able to leave the scene. I spoke with the operator of MV2 identified as Antonio Mendes Goncalves (S54275868) who stated he was stopped at the red light in the right middle lane on the Centre Street bridge. MV2 stated MV1 attempted to change lanes and struck the rear drivers side corner of the school bus. MV2 sustained minor damage and the operator reported no injuries. MV2 is a City of Newton contracted yellow school bus that had six children present on the bus at the time of the accident. The school bus was transporting th e children to Bigelow Middle School. All children present reported no injuries and their parents were notified by the school administration. I escorted the school bus back to the school where the children were all evaluated by the school nurse. 14 photographs were taken and submitted to the IT Bureau.

The operator of MV1 (NAKAWUNGU, Nisha) stated that she was traveling Southbound on West St. and pulled over the sidewalk to park on the front lawn of 78 West St. She stated once on the lawn, she must have hit the gas instead of the brakes, and the vehicle then proceeded forward to the right, crossing over West St. into the fence of 77 West St. The fence and bushes sustained damage and photos were taken and submitted to the IT bureau to be attached to this report. No injuries were reported and the vehicle was not towed from the scene. I responded to the area 107 Dartmouth St. at approx. 0345 on 04/28/9019 along with N493 Officer Kelly Donahue and N508 Sgt. Devine for a report of an MVA. The MVA was reported to be single vehicle crash. The vehicle left the roadway and was on the front lawn of 107 Dartmouth St. I arrived on location (first on scene ). I observed tire tread marks across the front lawn of 107 Dartmouth St. I then observed a vehicle in the bushes toward the left front side of the house. The vehicle; NY Reg. JHE8252, Porsche color blue. The RMV info on the plate came back registered to Porsche;of ;South. Registration plate type: Dealer Assignment. 185 W Sunrise HWY; Freeport,NY (no further info). The vehicle was unoccupied and secured. I felt the hood of the car which was still warm from the engine. I checked the immediate area surrounding the crash scene for an operator but found no one. I radioed to other responding units to canvas the neighborhood and surrounding area for the operator on foot. I spoke to the homeowners at 107 Dartmouth St., identified as Anthony and June Limberis. Mrs. Limberis stated she heard a load noise and a car engine revving. She stated when she looked out the window she observed a W/M tall, thin, wearing tan khakis and a light colored plaid shirt walking across the front lawn. He proceeded up Dartmouth St. (northbound) toward Commonwealth Ave. Newton Fire Dept arrived on location to check the crash scene and vehicle for possible leaking fluids and any other hazards. As I was talking with the firefighters we were approached by two men. One of the men was intoxicated. He was unsteady on his feet, had glassy bloodshot eyes, and was slurring his speech. The man was a W/M tall/thin approx. 6'2" and was wearing tan khakis and a light colored plaid shirt. He was identified as Eric Adams DOB 09/18/1969. The second man with him was identified as John Adams DOB 02/07/1936; An elderly W/M approx. 5'6" also wearing tan khakis and a light colored plaid shirt. Eric Adams stated his father John was the operator of the vehicle. Eric Adams had a strong odor of Alcoholic beverage emanating from his person and breath. He continued to slur his speech and was unsteady on his feet as he spoke. He was swaying back and forth and side to side as he spoke to me. Officer K. Donahue and Sgt. Devine were on scene at this time and also made the same observations. Multiple times we had to grab hold of Eric Adams to keep him from losing balance and falling. We spoke with Eric and John Adams for several minutes as their accounts of what took place were not consistent. I asked Eric Adams where the keys to the vehicle were. Eric Adams immediately pulled the starter key to the vehicle from his front right pocket. Eric Adams became defensive and belligerent when questioned about who was operating the vehicle. John Adams appeared tired (as if he just woke up) and confused as to the situation. Sgt Devine and I both spoke with Eric Adams and John Adams. Although both men claim John Adams was the operator of the vehicle, a clear determination was uncertain. A query of both mens driver's license status on NY Licenses came back active. Eric Adams has two prior DUI's on his RMV record. Officer Donahue and Sgt. Devine escorted the two men to 93 Dartmouth St. Eric Adams stated they were staying at that residence which is his sister's house. I stayed at the crash scene to document information and assist with clearing the MV from the property with Tody's Towing Service. As they were walking away I observed Officer Donahue and Sgt. Devine continue to have to assist Eric Adams with staying on his feet. Approximately ten minutes later as I was speaking with the aforementioned homeowners Mr.&Mrs. Limberis. Officer Donahue and Sgt. Devine returned to the crash scene. As they were updating me we observed Eric Adams returning to the crash scene. He was stumbling and swaying as he walked and repeatedly almost fell. Officer Donahue and Sgt. Devine immediately intercepted and assisted him before he fell. As they were assisting him again I continued speaking with the homeowners. Suddenly I heard Officer Donahue and Sgt. Devine struggling with Eric Adams who was now on the ground and resisting being handcuffed. I ran to assist them. Eric Adams continued to resist and be uncooperative until he was appropriately restrained in handcuffs. Eric Adams was placed under arrest by officer Donahue for Disorderly Conduct and Resisting Arrest. Reference Officer Donahues Newton Police Incident Report #19016484. The vehicle NY Reg. JHE8252 was removed from the property of 107 Dartmouth St. and towed from the crash scene by Tody's Towing Service.

On May 3, 2019 at approximately 18:18 I responded to a motor vehicle crash on Beacon St at the intersection of Hammond Pond Pkwy. It was reported that a school bus was involved with no passengers on board. Upon arrival, both vehicles were pulled over on the westbound lane of Beacon St. Both vehicles were turning left onto Beacon St. from Hammond Pond Pkwy, which has two left turn only lanes. MV1 was in the left lane, and MV2 was in the right. MV1 attempted to merge with MV2 on Beacon St. but misjudged the turn and drove into MV2's driver's side. MV2 was a school bus contracted by the City of Newton with no passengers on board. Photographs were taken and submitted to IT to be attached to this report. No reported injuries.

On Wednesday, May 29, 2019 while assigned to Traffic unit N525, I responded to the area of Centre Street and Washington Street, Newton for a report of a motor vehicle crash involving a school bus. The weather at the time of the crash was overcast and the road surface was dry. Centre Street and Washington Street are both public ways maintained by the City of Newton. The operator of MV1, Norma Cedeno (S60008138), stated she was operating a 2016 Dodge Caravan (MA SPN: 104893) around the Washington Street/Centre Street rotary. Cedeno stated as she was traveling in the left turn lane on Centre Street, she attempted to travel straight (N) through the intersection at Washington Street towards 255 Washington Street. Cedeno stated the front passenger side of her vehicle crashed into the driver side of MV2. MV2 was attempted to continue left around the rotary and was located on the passenger side of her vehicle. Cedeno reported no injuries. I observed minor damage to the front passenger side tire area of MV1. Cendeno's passenger, Cristina Velez (S04276767), reported not injuries. The operator of MV2, Mary Sanker (S62890912), stated she was operating her 2016 BMW 328XI around the Washington Street/Centre Street rotary. Sanker stated as she was continuing through the rotary in her lane, the vehicle traveling in the lane to her left (MV2) entered her travel lane in an attempt to travel straight towards 255 Washington Street and crashed into the driver side of her vehicle. I observed moderate damage to the front and rear driver side doors of MV1. Sanker reported no injuries. The operator of MV1 was cited for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care)

On Thursday, June 6, 2019 while assigned to Traffic unit N525, I responded to the intersection of Waverly Avenue and Franklin Street for a report of an accident involving a City of Newton school bus. The weather at the time of the accident was clear and sunny. The road surface was dry. Waverly Avenue and Franklin Street are both public ways maintained by the City of Newton. The operator of MV1, Dorothy Wensink (S40227061), stated she was operating her 2010 Toyota Prius (MA: 538EV6) Eastbound on Franklin Street towards Waverly Avenue. Wensink stated she came to a stop at the stop sign on Franklin Street (E) at Waverly Avenue. Wensink stated as she began to travel through the intersection, a school bus traveling on Waverly Avenue (S) crashed into the rear driver side door area of her vehicle. Wensink reported no injuries on scene. I observed moderate damage to the rear driver side area of MV1. Wensink was able to driver her vehicle away from the scene. MV2 was a City of Newton contracted yellow school bus (MA BUN: 39173) transporting students attending the Ward Elementary School. The bus was returning to the Ward School from a field trip. The operator of MV2, Antonio Goncalves (S54275856), stated he was operating the school bus Southbound on Waverly Avenue towards Franklin Street. Goncalves stated MV1 abruptly accelerated across the intersection from Franklin Street. Goncalves stated he was unable to avoid making contact with MV1 and crashed into the rear driver side of the vehicle. I observed minor damage to the front middle and passenger side bumper area of MV2. The following Ward Elementary School student, teachers, and parents were traveling on the school bus at the time of the accident and reported no injuries. Students: Avishai Arbili, Eleanor Chen, Maeve Donovan, Esat Efendigil, Mark Elentuck, Julian Ismay, Rhea Khanna, Griffin Kirby, Ori Layani, Priscilla Le-Ngo, Callum McLaughlin, Carolina Muniz, Emily Ney, Hrithik Ponduru, Levi Rosenberg Van Gameren, Zachary Schwarzberg, Ava Smoller, Matthew Somers, Lillian Welford, William Balerna, Alexandra Bogdanova, Randi Green, Christopher Iatrou Johnson, Charlotte Nichols, Henry Pozen, Sofia Shadah Alcalde, and Zoey Steinberg. Teachers: Katherine Goldman and Lauren Ludman. Parents: Enrique Shadah, Brian Rosenberg and Xinxin Wang. The Principal of the Ward Elementary School, Becca Brogadir, responded to the crash scene and signed a patient refusal for all of the students with Newton Medics. The students, teachers, and parents, then walked back to the Ward Elementary School with Principal Brogadir. The school bus was able to drive away from the scene. Once the students arrived at the Ward Elementary School, the nursing staff there evaluated each child before they could return to class. The school staff stated they would make notifications to the parents of the children involved in the accident. After speaking with all parties involved, the operator of MV1 was cited in hand for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Pictures were taken of both vehicles and submitted to the IT Bureau.

On August 1, 2019 at approximately 17:34 hours I, Officer Guarino, responded with Newton Fire and Medics for a report of a single vehicle crash into the building of 335 Boylston St. Upon arrival, the operator and sole occupant of the vehicle was out of the vehicle and had minor injuries. MV1 struck a stop sign and was up against 335 Boylston St, which appeared to have a broken window and minor structural damage. Operator of MV1 said that he was traveling in the right lane of Boylston St (Westbound) when a white Mercedes sedan (unknown model or registration number) turned into his lane and almost struck his vehicle. Operator of MV1 swerved to the right to avoid a collision and as a result crashed into the stop sign on John St. and the side of 335 Boylston St. Witness 1, who was parked at 325 Boylston St. saw the incident and corroborated MV1's story. A second witness, who I was unable to make contact with stopped and gave MV1 his phone number of (508)215-4951 and texted MV1 that he caught up with the possible vehicle, MA Reg 9BT373, which comes back to DESOUZA PLASTERING SERVICES INC. I will attempt to make contact with an operator during their business hours. The owner of 335 Boylston St, Onir Spiegel, was on scene, was given the incident number and was advised to contact her insurance company. MV1 was towed from the scene by Tody's and the operator signed a patient refusal from the Medics. Pictures were taken at the scene of the damage and submitted to IT to be attached to this report. Sergeant McLean notified the DPW for the stop sign and glass in the roadway. Sergeant McLean also left a message for Newton Inspectional Services to come inspect damage to the building at a later time.

Op bus# stated he did not strike the vehicle. He contacted his the shift supervisor to report the incident. -Next spoke to the resident of 702 Chestnut Street who exchanged information with the MBTA shift supervisor regarding the damage to the fence. - I observed damage to the right rear panel next to the bus's rear door. I observe green paint transference on the panel a result of striking the base of the traffic signal post. I next observe the post was leaning on the fence. I observed bends and dents on the surface of the top bar of the fence and paint chip offs. I took picture of the damage to the bus and the property involved. - There were no injuries reported at the time of the collision and no vehicle was towed from the scene.

On 08/13/19 at 14:53 hours, I responded to a hit and run motor vehicle accident located at 244 Needham Street. The operator of MV1 states she was parked in a parking spot to the right of the Splash business at 244 Needham Street. The parking spot is located on Charlemont Street. After parking, she was sitting in her motor vehicle when she heard a bump to her driver's side rear end from MV2 backing up from a parking spot across from her. She says the operator of MV2 turned and looked at her and kept proceeding (NB) on Charlemont towards Needham Street. She then exited her vehicle to try and get the operator's attention to no avail. She took a photo of MV2 which was a black Toyota Highlander bearing MA reg.# R85045. She described the operator as a white female in her forties with possibly dark curly hair. I observed minor paint damage and scratches to the driver's side rear quarter panel area. The operator is unsure if that damage is from the accident. The owner of MV2, Steven Masterson has a residential address of 40 Wild Acres Rd in North Attleboro, MA and a mailing address of 34 Central Street Foxborough, MA. A telephone search revealed a phone number of (269)857-7814 for him out of Aiken, SC and the Foxborough address but the number is not in service. UPDATE: I was provided a phone number for Masterson from N. Attleboro PD. On 08/16/19 at 12:45 hours, I spoke to Sheila Masterson who stated she was driving MV2 at the time of the reported accident. She stated that she was unaware that she struck MV1 and saw the other operator looking at her but thought nothing of it. Due to the accident being very minor or possibly no damage at all, no further action will be taken. Masterson was provided with the other operator's information.

Orama stated the vehicle behind him attempted to pass him on his right and the front passenger side of MV2 crashed into the rear driver side of his vehicle. I observed minor damage to the rear driver side bumper area. Orama reported no injuries. The operator of MV2, Michael Ansel (S11557140), stated he was operating his 20003 Mercury Marquis ( MA: 992SK2) near 2000 Commonwealth Avenue (W). Ansel stated he was having an emergency and was trying to get around traffic to use a restroom at a local business. MV2's front passenger bumper crashed into MV2's rear driver bumper area. Ansel reported no injuries. Ansel stated he was responsible for the accident to me.

On Tuesday, September 10, 2019 while assigned to Traffic unit N525, I responded to the area of Beacon Street and Roslyn Road, Newton for a report of a motor vehicle/bicycle crash. The weather at the time of the crash was overcast. The road surface was dry. Beacon Street and Roslyn Road are both public ways maintained by the City of Newton. I spoke with the bicyclist involved in the crash, Steven Greenzang (S96615185). Greenzang stated he was traveling Westbound on Beacon Street in the shoulder lane on his black Specialized bicycle. Greenzang had a yellow reflective vest on and was wearing a helmet at this time. Greenzang stated a vehicle in the travel lane to his left drove by him and hit the mirror located on his left handlebar. I observed damage to the mirror. Greenzang report no injuries. I spoke with the operator of MV1, Sarah Bonfanti (S66811372). Bonfanti stated she was parked in front of 1411 Beacon Street waiting to pick up a co-worker that resides at that address. Bonfanti stated a bicycle traveling on the sidewalk to her right crashed into the passenger side door mirror of her 2008 Toyota Scion (MA: 681YH1). Bonfanti stated the bicyclist was acting "crazy" during her interaction with him and he was trying to take her picture. Bonfanti stated she got back into her vehicle and moved up the roadway to wait for police to arrive on scene. I canvassed the area where the accident occurred with Sgt. Dempsey. There was glass from the passenger side mirror of MV1 and Greenzang's bicycle in the main travel lane of the roadway and the shoulder lane where Greenzang stated he was traveling. We also observed a large tree and a telephone pole on the sidewalk next to the roadway where the accident is alleged to have occurred. After speaking with all parties involved, observing the location of the glass in the roadway from the crash, and the location of the tree and telephone pole on the sidewalk where he accident occurred; Greenzang would not have been able to operate his bicycle on the sidewalk and be close enough to the roadway to make contact with MV1. Prior to clearing the accident, I asked Bonfanti if she had any questions and if she wanted to report any injuries. Bonfanti stated she was hit by the bicyclist during her exchange with him and began grabbing her left leg. I asked Bonfanti why she did not inform the officers on scene that she was assaulted. Bonfanti stated she said something to our Dispatch when she called. I asked Bonfanti if her leg was injured. Bonfanti stated I was putting words in her mouth and that she was just scratching her leg. Bonfanti then stated she was hit in the right shoulder. I asked Bonfanti if she would like an ambulance to respond and evaluate her. Bonfanti declined and stated she was more concerned about her mirror. I advised Bonfanti if she wanted to pursue charges against Greenzang she could do so in Newton District Court. I listened to the 9-1-1 recording Bonfanti made in regards to this incident. Bonfanti stated a pedestrian busted out her window. Bonfanti then stated to our dispatcher that the bicyclist lost control of his bicycle and smashed into her car. During her exchange with the our dispatcher, I did not hear Bonfanti say she was physically assaulted by Greenzang.

Vehicle #1 was parked legally on Cherry St and the operator has his own tow coming, as the vehicle is a contracted Waltham school bus van. Rizza stated he waited for the crosswalk signal to turn on. When it finally turned on to walk (white walking person) he got on his bike to start peddling. At this time, he observed a black vehicle traveling southbound traveling towards him. Moments later, Rizza was struck by the vehicle. Rizza rolled over the vehicle and hit the pavement. Rizza stated the vehicle took off, but he was unsure where it went (Mass Pike, Newton Corner, or Washington Street). The medics looked at Rizza and he was alert and had a minor laceration to his head. Rizza's mother Domenica Rizza picked him up and brought him to NWH. I drove around the area and could not find a black Honda sedan with damage to the front of it. As of now, I do not have anymore information. This report is a hit and run. Traffic Update (Officer Gaudet): Myself and Officer Anderson have both canvassed the area of the crash for identifying physical evidence from the vehicle that fled the scene with a negative result. We have also reached out to local business's in an attempt to secure surveillance footage of the crash with a negative result. At this time we are unable to obtain any further information on the vehicle that left the scene of the crash.

On 10/16/2019 at 1434hrs, I responded to Cherry St at Webster St for a two vehicle accident. The operator of MV1 (MA Reg: 519PW3) stated that she was travelling eastbound on Webster St and was proceeding through the intersection of Cherry St when the accident occurred. MV1 stated that she had the green light while going through the intersection. The operator of MV2 (MA Reg: 1BYL34) stated that she was on Cherry St travelling southbound and had a green light at the Webster St intersection. MV2 stated that she believed she had a green light and the accident occurred when she entered the intersection. MV1 sustained heavy front end damage. MV2 sustained passenger side damage. Both vehicles were towed by Tody's. The operator of MV1 was transported to NWH for further evaluation. After the initial collision MV1 was directed into the traffic light and bushes at the southeast corner of the intersection. Pictures were taken of the damage by Ofc. Ferguson. MV2 was issued Mass Uniform Citation T2079681 (c89/s9 - Failure to Stop Red Light). It should be noted that the accident caused damage to the bushes of the business at 432 Cherry St. I contacted the property owner, Dennis Cameron, and made him aware for insurance purposes.

On 11/12/2019 at 12:26 hours, I responded to the Star Market parking lot located at 2040 Commonwealth Avenue for a motor vehicle pedestrian accident. The parking lot is a public/thru way that motorists use to access the Star Market along with other businesses. The weather at the time was cloudy and rainy and the parking lot was saturated. Prior to my arrival, the pedestrian involved left the scene and did not want to speak to the officer nor wanted medical attention. Ofc. Claflin then cancelled Cataldo Ambulance. The pedestrian, Mary Alexander, left her information with a store employee prior to leaving. I spoke to the operator of MV # 1 (Joshua Randel MD Lic.# R534440603244) who stated that he was travelling through the third row of the Star Market parking lot at an appropriate speed. He was then attempting to make a left turn near the entrance/exit of the store. He says that there was a large white van parked in the end handicapped spot towards the left of him. He says the van was sticking out in the spot limiting his visibility. As he was half way through his left turn and coming around the corner, he was not able to see a pedestrian pushing a shopping carriage. He then struck the carriage that was being pushed by Mary Alexander. He further says that the cart jolted Ms. Alexander and she fell down to the ground. At the time, Alexander was walking through the lot near the store entrance heading back towards the parked cars. She was not in the crosswalk at the time which was approximately ten feet down from her location. After, I attempted to go to Ms. Alexander's residence to follow up. I spoke to her friend who was bringing up Alexander's groceries and she stated that she decided to go to Newton Wellesley Hospital for treatment. I then spoke to Ms. Alexander in the lobby of the hospital. She stated that she was exiting the store through the lot with her shopping carriage. She believes she was approximately eight feet away from the door when all of a sudden she only saw red coming at her. She believes MV # 1 was speeding at the time. I asked her if she was struck by the vehicle or was the shopping carriage only struck. She replied that she did not know and only remembers falling to the ground after. She sustained a large bruise to the inside palm area of her right hand and her left knee was swollen. Digital photos of the scene were taken and pictures of Ms. Alexander's injuries were taken at the hospital. As far as possible video surveillance, I was told that the Loss Prevention Department has to access the video. On 11/13/19 at approximately 14:20 hours, I went in to the Star Market and spoke to the manager, Nidal Rajeh, inquiring about possible video surveillance. He said there is coverage in the parking lot and he was able to show me the video. A copy of the video would have to be done by Loss Prevention which I requested. I observed at the 12:12pm mark in the video, MV # 1 was parked five spots up from the third row of the lot that he was exiting. I also observed a white transport van parked in the end handicapped spot mentioned by the operator but it was parked correctly and not limiting visibility. Next, MV # 1 backs out of the spot and drives forward going at a reasonable speed. The pedestrian (Alexander) has already exited the store with her carriage and is walking at an angle toward the same third row of parked cars. As the pedestrian is walking towards the direction of MV # 1, it appears that the pedestrian should be visible to him. MV # 1 then starts to take the left turn around the corner without slowing or stopping and strikes the carriage head on. Due to the impact, Alexander then loses grasp of the carriage and it goes past her twisting her body around. She then hits and rolls up on the hood of the vehicle on her left side. MV # 1 then suddenly stops and immediately gets out and provides assistance to the pedestrian. Due to the video surveillance, Mr. Randel will be issued/mailed MA Uniform Citation # T2012347 for Ch. 90/14/A Failure to slow for a pedestrian.

MV#1 was traveling North on Fair Oaks Ave, and attempted to make a right turn onto California St. MV#2 was parked in front of 589 California St facing West. MV#1 appeared to have run over the curb at the intersection, hit a city tree in front of 55 Fair Oaks Ave (California St side), and then crashed into MV#2. When I arrived on scene I observed a male party with blood on his head, and another male party standing next to him, later identified as Elmer Lopez.. I asked Elmer which car he was driving, and he stated that he was in MV#1. I asked Elmer what happened, and he stated that he and the other man with him, identified as Ricardo Deras, were in the rear seat with another male party, and that there were two other parties in the front seat, one of them was driving, he was not able to provide me with a name of who was driving. I asked him where everyone else was, and Elmer stated that after the crash, the got out of the car and ran away. At this point, medics arrived on scene and began to tend to Ricardo. I next spoke with the owner of MV#2, Jim Silva. Jim stated that he was in his home when he heard a loud crash. Jim stated that he came out of the house in time to witness the two males I was speaking with looking at the car, and he also stated that he observed three parties walk away from the scene, two heading east on California St, and one heading north on Fair Oaks Ave. MV#1 and MV#2 were towed off scene by Todys. Due to the scene, and the location of the cars in a busy street, I was not able to do a MV inventory of MV#1 until after it was towed by Todys. The inventory was done at Todys Service station, and a MV inventory sheet was filled out and placed the in appropriate box. I took three pictures of the damaged tree and submitted them to IT to be attached to this report. While I was taking the pictures of the tree, two males were walking past the scene. I stopped both parties and questioned them about the incident. They stated that they had been in the car, but had not been driving. I asked them if the knew who the driver was, and they told me that they did not know the name of the driver. Due to no witnesses observing the driver and all parties stating that it was someone else, I was not able to determine a driver of the vehicle. Units checked the area for the fifth party in the vehicle with negative results. Ricardo was transported to NWH hospital by medics. All other parties were sent on their way. All units cleared without further incident. **UPDATE** On Nov 25, 2019 at approximately 1621 hrs, I contacted Westborough Police Department in an attempt to get a hold of Dora. Westborough informed me that they made an attempt to contact her with negative results. Westborough PD left my contact information for her to call me back.

The operator stated that she was traveling southbound on Waltham St when MV2, which was parked facing southbound in front of Highlander Farms, abruptly pulled out and hit her. The operator of MV2 then pulled a U-turn and fled northbound into Waltham. MV2 was described as a small light grey car with drivers side damage and a popped front right tire. The operator of MV2 is believed to be female. Waltham PD was notified and searched the area with negative results. There are no exterior cameras on any of the businesses in the area nor was I able to locate any witnesses. No injuries were reported and MV1 was driven from the scene.

On Monday, December 9, 2019, while assigned to Traffic unit N525, I responded to the intersection of Lincoln Street and Walnut Street, Newton for a report of a motor vehicle crash involving a pedestrian. The weather at the time of the crash was rain. The road surface was wet. Lincoln Street and Walnut Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jill Edelson (S99137352). Edelson stated she was stopped in traffic while operating her 2012 Toyota Camry (MA: 81ZH83) Northbound on Walnut Street at Lincoln Street at a red light. Edelson stated the light turned green and she had a green left turn only arrow. Edelson stated she turned left into Lincoln Street from Walnut Street when the front passenger side of her vehicle crashed into a pedestrian in the roadway. Edelson stated she did not see the pedestrian prior to the impact. I observed damage to the lower portion of the passenger side windshield. Edelson reported no injuries. I spoke with the injured pedestrian, Lawrence Murren, at Brigham and Women's Hospital. Murren stated he had entered the crosswalk on Lincoln Street at Walnut Street and does not remember if he had the pedestrian signal. Murren believes MV1 did have the green light. Murren stated after he entered the crosswalk he looked back towards the "59 Bus". Murren stated he then looked forward and saw MV1 was in front of him. Murren stated he was unable to avoid the vehicle. Murren stated he has an injury to his hip and had a head laceration. I spoke with a witness to the accident, Edward Austin. Austin stated he was in the vehicle behind MV1 at the traffic light on Walnut street. Austin stated the traffic light turned green and MV1 had a green left turn arrow. Austin stated MV1 started to go and make their left turn. Austin stated he observed an older gentleman starting to walk out into the street. Austin stated the pedestrian had his head down and had an umbrella due to the rain. Austin stated the pedestrian also had a do no walk sign. Austin stated MV1 hit the crossing pedestrian in the crosswalk. Austin stated he does not believe either saw each other. Photos were taken of the area and submitted to the IT Bureau.

MV1 stated his dash cam recorded the incident showing a green and yellow colored Prevost bus with FLIXBUS in large lettering make contact with his vehicle. MV1 stated he already filed an accident report with the RMV and notified his insurance company. I attempted to make contact with the FLIXBUS company to get the drivers contact information but was only able to speak with a customer service representative in a foreign country who would not provide me with any information. I was able to locate an email address for the company and emailed a request for information but have not received a response. I advised MV1 of my investigation results and he was satisfied. MV1 stated the damage to his vehicle was minor.

The operator of MV#1 stated he was travelling northbound on Walnut St when the top portion of his vehicle caught some low hanging wires and took down 2 utility poles and damaged a third utility pole. There were no reported injuries to the operator of MV#1 and no visible damages to MV#1. The operator of MV#1 is a delivery driver that is familiar with the area and frequents the same route. There were several live primary wires down on Floral St and Walnut St. Unit N-498 Officer Raymond blocked off Walnut St at Centre St, Unit N-502 Officer Nardelli blocked off eastbound Floral St and I blocked off Walnut St at Lincoln St. The surrounding pedestrian sidewalks were all taped off with Police caution tape. MV#1 struck the low hanging wires on Walnut St at the intersection of Floral St. Utility pole #A (47/12 ) located on Walnut St directly in front of the parking of the "Orchid Dry Cleaners" at 1175 Walnut St was cracked at the base. Utility pole #B was located on the westbound side of Floral St was split at the base and was dangling over the parking lot at 1175 Walnut St. Utility pole #C was located on the eastbound side of Floral St and was completely removed from its foundation and was now leaning on the building at 1182 Walnut St. The street/ stop sign of Floral St was damaged with the "Stop" sign and the "Floral St" sign completely removed from the pole (Located on the eastbound side of Floral St). A city tree limb that was damaged and landed on top of parked Vehicle #2 (Parked on the eastbound side of Floral St across from 49 Floral St. Wisconsin registration 850TGM). The owner of vehicle #2 was informed of the damage. Traffic Officer Gaudet took digital photographs of the damaged street/ stop sign and the damaged city tree. The witness Pat O'Hara of 1185 Walnut St stated MV#1 did not cause the accident but a previous bigger truck did. All he could provide was that the vehicle was a big truck travelling northbound on Walnut St. The witness Edna Jacquet stated she was parked on Walnut St (Parked between 09:00 to 09:15) at the MBTA bridge facing southbound when she observed the accident. Edna stated she observed the operator make eye contact with her and she proceeded to honk her horn in attempts to stop him. Edna stated the vehicle that caused the accident was a white colored 18 wheeler type of truck with the markings of "3 letters" all in different colors. Edna have a company video camera that faces forward attached to her vehicle that may have captured the accident. I spoke to her company manager Lisa Sanella at "TransAction Corporate Shuttles" located at 5 Wheeling Ave in Woburn MA and she will notify me if she is able to view video footages. The building owner of 1170- 1176 Walnut St Stas Gavronsky have a surveillance camera that faces Walnut St southbound towards Floral St. He will review the video footages once power have been restored that the area and notify me. On 01/07/2020 Lisa Sanella contacted me stating that they were able to review video footages from the front facing camera of Edna's shuttle bus. She stated that the involved vehicle was described as a big white colored 18 wheeler type of truck and did not have a front registration plate but did display the commercial company name of "GHG Logistics". Lisa stated the accident was time recorded at 09:13. Lisa made a copy of the video footage and will attempt to email it or deliver a flashdrive to me. An internet search of GHG Logistics showed that it is a distribution company out of 6390 Hedgewood Dr suite #300 out of Allentown PA. This company distributes wholesale sushi to the northeast area and their delivery trucks are equipped with "Rydsmart" GPS tracking devices enabling them to know their locations at all times. I spoke to Joanna Gerwig who is the store support team leader of the "Wholefoods" located at 916 Walnut St inquiring about a possible sushi delivery and she confirmed that GHG Logistics does regular deliveries and did deliver items yesterday morning between 08:00 -09:00. She stated that the truck displayed trailer #250634-481801 and that the driver was described as a big sized white male by the name of "Will" with a contact number of (570) 350-3453. She also have video footages of the truck making the delivery and provided me with their contact number of (800) 668-7700 and their corporate number of (484) 223-4502 (Confirmed to be the same company as my internet search). I viewed the video footage from Transaction Corporate Shuttles and it showed the 18 wheeler truck was travelling northbound on Walnut, catching the utility wires above and causing an electrical spark on the eastbound side of Floral St. It showed the truck slowed down and then continued to travel northbound. I spoke to Will at (570) 350-3453 and he confirmed he was making deliveries to the Wholefoods and was on Walnut St at approximately 09:00. Will stated the top corner portion of his truck did catch the above wires and heard a "twang". Will stated he briefly stopped and did not observed any damages so he continued to make his deliveries. Will stated he have been making the same delivery route for 2 years and never had an issue with low hanging wires. Will's full name is William L. Gray and have a Pennsylvania driver's license # 24551299. The truck's registration is apportioned Indiana 2917618. I have forwarded the video footage to the I.T bureau and it was attached to this report.

At approximately 1645HRs on Tuesday, 01/07/2020, I was dispatched to 325 Boylston St. (Sunoco Gas) for a report of a hit and run motor vehicle crash. Upon arrival, I spoke with the operator of MV1 (MA Veteran:J706), who stated that as he sat stationary in his vehicle just before entering pump: 3, MV2 backed up from pump: 1 and struck MV1 in the front bumper, despite him activating his horn and keeping it active as she continued to back up. The operator of MV1 then approached the driver's side door of MV2 to exchange information, however the operator of MV2 drove off; the operator of MV1 provided MA Pass: 8SV842 being attached to MV2 and I noted damage to the front bumper of MV1. I spoke to witness 1 via phone and he concurred with the operator of MV1's account of the incident. It should be noted that witness 1 could see the crash well from his vantage point by pump: 2. The operator of MV2 could not be reached by this officer for comment. Natick PD responded to the operator of MV2's residence and observed damage to MV2's rear bumper. They requested the operator of MV2 contact me via phone and she stated she would; Natick PD did not get a contact number for her. Natick PD informed us to expect her call and a few minutes later, dispatcher Kelly stated that she received a call from the operator of MV2, who immediately hung up once the call was answered. It should be noted that since she called on the business line and immediately hung up, we were not able to retrieve her phone number as the system does not store it. Due to the operator of MV2 failing to provide her information after the crash/fleeing the scene and her not being cooperative with my investigation, I have issued her by mail MA Criminal Citation:T2013367 for violating one count of MGL. Ch. 90 S. 24 (Leaving The Scene Of An Accident After Property Damage). Note: the clerk at Sunoco stated that the cameras at the pumps do not store footage. **Update** At approximately 1540HRs on Thursday, 01/07/2020, I spoke with the operator of MV2 via telephone. She contacted the Newton Police Traffic Bureau and provided her phone number, who in turn forwarded it to me. She stated to me that on the night in question, she was at the pumps and the pump she initially wished to use (pump: 1) had a sign on it to pay inside. She then elected not to use said pump and backed her car up and heard the operator of MV1 activate his horn. She stopped and was unaware that she struck MV1. When she saw the operator of MV 1 exit his vehicle, she assumed he was going to pump gas. She then stated she left the gas station because she was frustrated and did not intend to leave the scene of an accident. I advised the operator of MV2 that since I was unable to speak with her that night, I had already issued her MA Criminal Citation:T2013367 for violating one count of MGL. Ch. 90 S. 24 (Leaving The Scene Of An Accident After Property Damage) via mail and advised her how to proceed.

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

On 1/29/20 at approximately 1835 hrs I responded to 50 Winchester St for a report of a MVA hit and run. Upon arrival I spoke with the owner of Tile in Style which is located at this address. Inga Sipicina stated that at 1050 hrs a brown UPS truck bearing S79977 struck a sign that was attached to the building above the entrance in two different locations. Inga stated the truck then took off. The sign had clear but minimal damage. A branch manager from UPS later came to the store to speak with Inga and her husband Slava and stated they would be in contact with them but didn't leave any information. I attempted to call the business but it was after hours and left a message. There were no injuries involved in this accident. On 2/4/20 I again attempted to contact UPS with negative results.

On Friday, February 7, 2020 while assigned to Traffic unit N525, I responded to the rear parking lot of 1 Boylston Street, Newton for a report of a hit and run motor vehicle crash. I spoke with the operator of MV1, Hong Zhang (S39408212). Zhang stated while operating her 2019 Volkswagon Tiguan (MA: 6YJF90), she was attempting to back out of a parking spot in the parking lot behind 25 Boylston Street, Newton. Zhang stated while backing out of the spot she crashed into an unknown vehicle traveling Westbound through the parking lot. Zhang stated the only information she had for the other vehicle was that it was a large red and black truck. I observed damage to the rear trunk window of MV1. Zhang reported no injuries. Zhang stated she was unsure if she hit the vehicle or if something fell off of it as it was passing by. I went to several local businesses in the area in an attempt to view surveillance footage of the area where the accident occurred with a negative result. While canvassing the area for the other vehicle involved, I observed a red dump truck (MA CON: T71619) removing snow on the property next to Hammond Pond Parkway. The operator stated he has been working down that end of the parking lot for most of the morning and stated he was not involved in a crash. I observed no fresh damage to the vehicle consistent with a crash.

On 02/26/2020 I responded to the area of Eastbourne Rd and Ireland Rd for a motor vehicle crash involving a pedestrian. Eastbourne RD is a public way which runs from Ward St north bound intersecting with Ireland Rd on the far north end. Ireland RD is a public way which runs west to east. Both the east end and the west end of Ireland Rd are dead ends. The crash occured on Ireland Rd's east end near the intersection of Eastbourne Rd. The crash involved Vehicle 1 described as a 2016 Acura SUV color grey bearing MA passenger registration 3RB127. Vehicle 1 had a valid inspection sticker which expires on 08/30/2020. Vehicle 1 was operated by its owner identified as Barbara Ball. Ball has an active MA Operators License (S18727836). The involved pedestrian was identified as Bruce Terrio. Terrio is an employee of Feeney Brothers Utility Services. Feeney Brothers Utility Services is a contractor of National Grid Gas Company. Upon arrival I observed the following: The crash occurred in the daylight hours around 10:30am on Ireland Rd near the intersection of Eastbourne Rd. At the time of the crash the weather conditions were clear, 48 degrees Fahrenheit and overcast skies (weather.gov). The area where the crash occurred was an active construction site for Feeney Brothers Utility Services. The area of the crash is a dead end. The area is accessed from Ward St from either Eastbourne Rd or Coolidge Rd. The entrances of both Eastbourne Rd and Coolidge Rd had construction signs indicating utility work in the area. At the time of the crash, all roadways were open to vehicle traffic. On the eastbound side of Eastbourne Rd just prior to the intersection of Ireland Rd, I observed a large air compressor which was parked in the roadway facing south bound. No traffic cones were located near the air compressor. To the right of the air compressor were thick green bushes. A wooden fence was located to the right side of the green bushes. On the east side of Ireland Rd I observed a large excavated hole on the right side of the roadway. The hole was near the intersection of Eastbourne Rd and was located approximately three feet from the curb. I observed four traffic cones, two on the front side and two on the left side of the hole. Located on the front edge of the hole was a tire print. Vehicle 1 was located on the west end of Ireland Rd. The front passenger tire of Vehicle 1 had fresh dirt located on the sidewalls and the tire tread. Vehicle 1 appeared to have no visible damage. The pedestrian (Terrio) was being treated by EMS and Fire for injuries. Terrio was wearing brown pants and a green reflective safety vest. I spoke with the operator of Vehicle 1 Barbara Ball. Ball stated that she was operating Vehicle 1 northbound on Eastbourne Rd near the intersection of Ireland Rd. Ball stated that she made a right turn onto Ireland Rd and drove on to the edge of the excavated hole. Ball stated that her vehicle then made impact with Terrio who was in the hole. Ball stated that she immediately stopped, backed out of the hole and called 911. Ball stated that prior to making a right turn onto Ireland Rd, there were no traffic cones in the roadway or next to the hole. Ball stated that as she made the right turn onto Ireland Rd she did not observe Terrio or the hole until it was too late. Ball states that she resides at 80 Eastbourne Rd which is the residence located to the immediate right of the crash location. Ball states that she had been recently parking on Ireland Rd due to ongoing construction at her residence. Ball states that she was traveling at a slow speed prior to impact. At the time of the crash there were three other Feeney Brothers employees working at the site. These workers were identified as Steven Gamble, Mike Cabrinma and Gerard McComaskey. At the time of the crash all three workers were located further up Eastbourne Rd near the intersection of Coolidge Rd and did not observe the crash. At the time of the crash, Newton Police Sergeant Glenn Chisholm was working a paid detail for Feeney Brothers. Sergeant Chisholm stated that he did not observe the crash.Terrio was transported by Cataldo EMS for minor injuries. Several photos were taken of the scene. Photos were taken of the hole with traffic cones and without traffic cones. All photo's were submitted to the IT Bureau. After clearing the scene, I responded to St Elizabeth's Hospital in Brighton, MA to interview Terrio. Terrio stated that at the time of the crash he was standing in the hole on Ireland Rd performing gas utility work. Terrio stated that the front passenger side bumper of Vehicle 1 struck him in the right shoulder. Terrio stated that the impact caused him to lose his balance and fall down in the hole. Terrio states that after falling down he was able to climb out of the hole without assistance. Terrio stated that at the time of impact he was wearing a white hard hat which he thinks struck the pavement on the edge of the hole. Terrio states that at the time of the crash he was working alone in the hole. Terrio states that at the time of the crash traffic cones were located near the hole. Terrio stated that as a result of the impact he sustained minor injuries to his right shoulder and back. I then observed Terrio's hard hat which appeared to have a fresh scrape mark on the top. The hard hat also had broken interior straps. Terrio stated that the straps broke when he was struck by Vehicle 1. Photos were taken of the hard hat and were submitted to the IT Bureau. After reviewing all facts of this incident, Vehicle 1 appeared to have been traveling at a slow speed prior to the crash. This was evident due to the lack of tire skid marks prior to the tire print on the edge of the hole. It also appears that Ball's view of the hole and Terrio may have been obstructed by the parked air compressor on Eastbourne Rd. The thick green bushes and a wooden fence to the right of the air compressor also added to the obstructed view. The hole was also located in close proximity to the air compressor which reduced Vehicle 1's reaction time. Jeff Oliver of OSHA was also notified of this incident. On Friday, March 6, 2020 while assigned to Traffic unit N525, I responded to the area of the Countryside School (191 Dedham Street, Newton) for a report of a motor vehicle crash involving a City of Newton contracted school bus. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with the operator of MV1, Gianna Gemelli (S95586165). Gemelli stated she was traveling Southbound on Parker Street towards the Boylston Street (E) on ramp in a JSC Transportation owned 2016 Dodge Grand Caravan (MA SPN: 105641). JSC Transportation is contracted by the City of Newton to transport students through out the city. Gemelli stated as she was traveling on Parker Street, MV2 attempted to pass her on her driver side to take a left onto Boylston Street (E). Gemelli stated the front passenger side of MV2 made contact with the rear driver side of MV1. I observed minor damage to the rear driver side bumper area of MV1. Gemelli was transporting two students to the Countryside Elementary School at the time of the crash. The students in the vehicle were identified as Mayson Martinez and Hailey Nesvacil. No injuries were reported by any of the occupants of MV1. Countryside Principal Elizabeth Herlihy signed a patient refused for Mayson Martinez. Martinez's mother Rosa Mordan was notified of the crash by school staff. Gemelli signed a patient refusal for her daughter, Hailey Nesvacil. Regina Moody was notified of the crash. I spoke with the operator of MV2, Clary Binns (s100112165). Binns stated she was traveling Southbound on Parker Street towards the Boylston Street (E) on ramp. Binns stated she attempted to go around MV1 to enter the on ramp to proceed onto Boylston Street when the front passenger side of her vehicle made contact with the rear driver side of MV1. I observed minor damage to the front passenger side fender/wheel area of MV2. Binns reported no injuries. Prior to my arrival, Binns stated she had two passengers in her vehicle at the time of the crash that had left the scene. They were identified as Maya Bock and Jonatan Freedman-Naditch. I contacted parents for both parties to notify them of the crash. No injuries were reported. Binns, Bock, and Freedman-Naditch are all students the Meridian Academy in Boston. Clary Binns was provided with Massachusetts Uniform Citation T0115909 for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Photos where taken of both vehicles and submitted to the IT Bureau. Parker Street and Boyston Street are both public ways in the City of Newton.

Operator of veh#1 stated he was parked inside the lot of #300 Needham St. in a parking spot. When he attempted to back out, he forgot this vehicle was in drive when he gave it gas. He then struck some bushes and a metal fence surrounding the property. The operator then panicked and drove home. Once at home he realized he should go back to the scene and was on his way back when the Police contacted his father. The operator and father showed up on scene and gave me the proper information. I gave the operator of verbal warning for leaving the scene and advised him that if he had not come back on scene there would have been a criminal citation issued. He and his father thanked me and left without any further issues. Operator #1 states that he was taking a left onto Lexington St from Wolcott St when his vehicle (vehicle #1), MA 1ZDP33, was hit by a white Chevrolet pickup with unknown plates. Operator #1 states that he had a had a green light but was able to swerve out of the way to minimize contact when the white truck ran the red light southbound on Lexington St. Damage is to the front right corner of vehicle #1. The white pickup did not stop. It turned right onto Commonwealth Ave towards Weston. I spoke to witness Sean Murphy (508-661-9329) who heard the crash and observed the immediate aftermath of the pickup taking off. He provided a further description of a 2 door white Chevrolet pickup, early 00's model, occupied 2x, with the name Empire Concrete on the passenger side. I attempted to locate an Empire Concrete, however limited search results were available. A possible business address of 54 Forest St Mllford was found and I had Milford PD respond to the address to look for the pickup. It was not there, and I was informed that the address itself is an apartment complex. It should be noted that there is no phone number or website for an Empire Concrete out of MA, although there are several other businesses that have the word Empire in the company name. The camera facing Lexington St at Gregorio Jewelers (corner of River and Lexington) did not yield any matching results for the appropriate time frame. Starbucks (corner of Commonwealth and Lexington) doesn't have outdoor surveillance equipment. Vehicle #1 did not need to be towed and no medical attention was requested by the operator. He was advised that if I am able to obtain further information or verify the company, I will contact him.

On August 31st, 2020 at approximately 09:33 hours while working N491 I responded to Adams Court for a reported two vehicle crash. On my arrival I first spoke with Officer Pelligrine who reported that a tenant of his identified as Alexandra Locke had her parked car struck by a delivery truck. I then spoke with Alexandra Locke(owner vehicle #1) who stated that her parked MV was backed into by vehicle #2(Anheuser Busch delivery truck) witnessed by her landlord Jim Pelligrine. (damaged to her passenger side rear). I then spoke with the operator #2 who reported that he had drove straight down Adams Court from Adams St and was backing up to go towards his delivery area for Dions liquor store on Watertown St when he accidently struck MV#1 which was parked at the time. Both swapped information. There were no tows or any injuries from this accident.

On 9/1/20 while working N493, I responded with N497 and the Fire Dept to 40 Crescent St for a report of a car into the house. On arrival I observed MA 6CM450 in the backyard of #40 unit C. It was stationary against the large AC Compressor in the rear of the unit, and appeared to had run over several large shrubs before coming to a stop. I spoke with vehicle owner and operator Marion Cohen 8/27/46. Cohen stated that as she was parking in the rear lot of the complex, she accidentally mistook the brake for the gas pedal. She in turn ended up going over the curb and through some bushes before hitting the compressor. There was a passenger in the vehicle identified as Milton Kornfeld 3/10/42. Both occupants were checked out by medics and declined transport. The airbags did not deploy. There does not appear to be any structural damage to anything other than the compressor. NFD checked the integrity of the house, and the compressor was unplugged. I took pictures of the scene and submitted the memory card to IT. Homeowner Xiaozhi Zhao and his family were present inside at the time of the collision and are unharmed. Tody's responded and removed the vehicle from the property. It sustained heavy front end damage and was towed to their lot via flatbed. A m/v inventory form was filled out. This report number has been provided to all parties for their respective insurance companies.

The operator of MV#1 stated he was travelling southbound on Walnut St when he observed MV#3 pull out of the driveway of Walgrens (Located on 1101 Beacon St but the driveway goes out onto Walnut St), struck MV#2 who was travelling northbound, pushing MV#2 into MV#1. MV#1 is an MBTA bus. MV#1 sustained minor damages to its driver's side front bumper area. There were no reported injuries to the operator of MV#1 or its passengers. All of the passengers onboard of MV#1 left the area prior to my arrival except for 1. The operator of MV#2 stated she was travelling northbound on Walnut St when she was struck by MV#3, which in turn pushed MV#2 into MV#1. MV#2 sustained heavy front end damages with driver's side airbag deployment and heavy driver's side door panel damages. The operator of MV#2 sustained injuries to her neck and shoulders. She was transported by Newton Paramedics to Newton Wellesley Hospital. The operator of MV#2 had a small dog with her that was located in the front passenger seat. Newton Animal Control took possession of the dog and transport it to the veterinarian. It was reported that the dog sustained possible internal trauma. The operator of MV#3 stated he was pulling out of the driveway of Walgrens onto Walnut St and struck MV#2. The operator of MV#3 stated MV#2 was travelling at a high rate of speed. MV#3 sustained heavy front end damages. There were no reported injuries to the operator of MV#3. Tody's Towing took possession of MV#2 and MV#3. MBTA Police Officer Michael Bolotin arrived on scene and documented the accident (MBTA case #2020-5909). MBTA Officers took possession of MV#1. The City of Newton DPW's street sweeper cleaned the debris on the street.

On 09/30/2020 at approximately 19:32hrs members of the Newton Police Patrol Bureau responded to a motor vehicle crash near the intersection of Washington St and Eddy St. After an initial investigation of the crash the Patrol Bureau secured the crash scene and called in the Newton Police Traffic Bureau to investigate. At approximately 19:45hrs Sgt. Devine, Ofc Wilson and I (Ofc Schlegel) from the NPD Traffic Bureau responded to the scene to investigate. Upon arrival to the scene I was met by Ofc Kane of the NPD Patrol Bureau. Ofc Kane stated that the crash was located in the driveway of 74 Eddy St and that it involved a Ford F350 pickup striking two parked cars. Ofc Kane stated that the operator identified as Kyle Butler had already been taken to Newton Wellesley Hospital by Cataldo EMS for injuries. . Upon approaching the crash scene I observed the following: The crash occurred at night time hours at approximately 19:32EST near the intersection of Washington St and Eddy St. Both Washington St and Eddy St are public ways in the City of Newton. The weather conditions at the time of the crash were a dark and clear with a temperature of approximately 60 degrees F. Washington St is a four lane undivided roadway (2 westbound travel lanes and 2 eastbound travel lanes). Washington St lighted roadway which has concrete light poles on both the westbound and eastbound sides. Washington St was under construction at the time and had a base coat of asphalt applied. At the time of the crash Washington St did not have any painted lane markings due to ongoing construction. Washington St is separated from the road sides on both the eastbound edge and the westbound edge with fixed granite curbing. The roadway also had several orange traffic barrels and traffic cones located edges of the eastbound and westbound lanes. Affixed to the light poles and the traffic barrels were temporary no parking signs for the ongoing road construction. At the time of the crash no road construction activities were taking place. No major debris or other obstructions were observed in the roadway. Washington St is a major roadway in the City of Newton and travels west to East across the entire City of Newton. This area of Washington St where the crash occurred is in between West Newton Square and Newtonville Square, this area has a posted 35 MPH speed limit. Both West Newton Square and Newtonville Square are densely populated with several businesses and residential areas. The section of Washington St where the crash occurred is also traveled frequently by pedestrian, bicycle and motor vehicle traffic. At the time the crash occurred 19:32hrs This area of Washington St is still a heavily travelled roadway. I observed Vehicle 1 described as a 2003 Ford F350 color white (MA Reg 5DT664) located in the driveway of 74 Eddy St. 74 Eddy St is a multi-family dwelling house located on the corner of Washington St and Eddy St. The driveway of 74 Eddy St is on the Washington St side. Vehicle was faced easterly across the driveway. I observed Vehicle 1to have extensive damage to the entire vehicle including but not limited to the enclosed cab area, cab roof and all windows, front end, rear end, along with both driver and passenger side. I also observed city of Newton owned concrete traffic light resting on top of the cab area of Vehicle 1. Upon further observation of Vehicle 1 it appears that both front tires and rims were flat, heavily damaged and had little to no tread left. Both front tires were worn down to the interior steel belt. I also observed that Vehicle 1 did not have a valid inspection sticker. Located to the immediate front of Vehicle 1 and resting on the front end was a white fence which had damage consistent with the impact of a motor vehicle. Immediately to the driver's side of Vehicle 1 was Vehicle 2 described as a 2007 gray Subaru Forester (MA Reg 45D610. Vehicle 2 was facing westerly across the driveway. Vehicle 2 had extensive damage to the driver's side front end, rear end, in addition to the entire driver and passenger sides. At this time Vehicle 2's drivers door was in contact with Vehicle 1's drivers door. Both drivers doors could not be opened.The damage to Vehicle 2 appeared to be consistent with impact from another motor vehicle. To immediate passenger side of the Vehicle 2 I observed Vehicle 3 described as a 2011 Toyota Rav4 color black (MA Reg 47XB99). Vehicle was facing in an easterly direction across the driveway. I observed the Vehicle 3 to have extensive damage to driver's side rear quarter panel, passenger side front quarter panel and front end. Also observed was damage to south east corner of the dwelling of 74 Eddy St. This area of the dwelling is located adjacent to the drive way. The damage appeared consistent with the impact caused by a motor vehicle. Also observed were large amounts of debris located in the area of the crash which included the sidewalk, yard and the driveway of 74 Eddy St. The debris included pieces of concrete which appeared to have been dislodged from the concrete light pole. A city of Newton no parking sign was lying in the yard of 74 Eddy St. This parking sign would have been attached to the concrete light pole due to ongoing road construction on Washington St. I also observed two tire marks on a curb stone located next the westbound lane of Washington St. These tire marks were in front of the residence of 74 Eddy St and were to the immediate left of a storm drain. The tire marks were consistent with being struck by a motor vehicle. In between the tire marks and just after the curb there was a hole in the ground with wires sticking out of it. This hole appears to be the location the concrete light pole was located prior to impact. Tire marks were located on Washington St traveling from the east bound lane easterly across the westbound lane and ending at the tire marks on the curb. The total length of the tire marks was approximately 190 feet. After conducting an on scene investigation it appears that Vehicle 1 from the start of the tire mark in eastbound travel lane of Washington St traveled 190 feet in the easterly direct across the westbound lane of Washington St and impacted the curb in front of 74 Eddy St. The tire marks had lateral striations almost the entire length indicating that the front tires were rolling and sliding with little to no braking prior to impact with the curb. After impact with the curb Vehicle 1 exited the roadway, struck a concrete light pole. Vehicle 1 then traveled over a sidewalk, across the yard of 74 Eddy St and into the driveway of 74 Eddy St. Located in the driveway of 74 Eddy St were Vehicle 2 and Vehicle 3. Vehicle 2 was parked facing toward the rear of 74 Eddy St. Vehicle 3 located to the passenger side of Vehicle 2 and was parked facing towards Washington St. When Vehicle 1 entered the driveway it struck Vehicle 2. The force of the impact of Vehicle 1 striking Vehicle 2 caused Vehicle 2 to strike Vehicle 3. The force of the impact of Vehicle 2 striking Vehicle 3 caused Vehicle 3 to strike the southeast corner of the dwelling located at 74 Elliot St. Vehicle 1 continued across the drive way and struck a white fence before coming to final rest. Vehicle traveled a approximately 55 feet from when it initially struck to the curb till final rest. No other injuries were reported on scene. All involved vehicles were towed from the scene by Tody's Towing. After the investigation the operator of Vehicle 1 Kyle Butler was issued Citations T2015882 and T2015883 for: Mgl 90/24/O Recklass Operation of a Motor Vehicle Mgl 89/4B/A Failure to Operate in the Right Lane Mgl 90/20 Failure to Submit a Motor Vehicle for Inspection Mgl 90/7 Defective Equipment (worn front tire treads) Mgl 90/13A Failure to wear a seatbelt An immediate threat form was also filed with the MA RMV. On Wednesday October 28, 2020 at approx. 0900 I responded to 24 Maguire Court for a report of a crash with property damage involving a tractor trailer. Upon arrival I met with the operator of the tractor trailer Mr. Williams, he stated he was traveling on Maguire Court delivering equipment to a local business, and while turning into the business parking lot he misjudged the turn and the trailer struck a fence,posts,wall belonging to 24 Maguire Court. I advised the owners son who was on the property of the crash and advised I would have a report. There were no reported injuries and all parties advised of the process. I also conducted a Level one inspection on the truck/trailer. MV1 stated she was attempting to manuever around vehicles to exit the street when "I stepped on the gas and not the brake" while in reverse subsequently going backwards, over a curb, across the lawn, and into the bushes of 17 St James Ter sustaining heavy damage throughout the vehicle. The homeowner of the residence reported damage to his curb, lawn, bushes and electrical conduit (Newton Fire checked the residence prior to clearing). Todys Towing responded and removed the vehicle from the lawn and leaving it on the street as the owner wanted AAA to respond. Due to airbag deployment, Fallon Ambulance responded to the scene, however the driver refused to be evaluated (also refused to sign a refusal).

However, due to the snowy and slippery road conditions, MV1 operator stated she was unable to stop. MV1 operator entered Lexington St and sideswiped (opposite directions) MV3. MV2 jumped the right side curb, sideswiped a tree and ended in the bushes of 236 Lexington St. MV2 sustained front end damage to the MV and plow, and driver side (no tow required). MV3 operator stated she was operating southbound on Lexington St when she obserbed MV1 enter Lexington St. However, due to the weather conditions, MV3 was unable to stop in time and sideswiped MV1 (opposite direction) and sustain front bumper damage (no tow required). On Friday 2/5/21 at approximately 0457 hours while assigned to marked unit n499 I was dispatched to 300 Boylston Street (Lifetime Fitness) for a report of a vehicle that drove into the security gate at the entrance to the parking garage. Upon arrival I spoke with SLOSS Mark (Security Officer) who stated a member of Lifetime Fitness was entering the parking garage and did not see the security gate down and drove into it. Impact caused the gate to bend in several areas and rendedered it inoperable in the down position. SLOSS contacted building maintenance and was awaiting their arrival. SLOSS further stated he works 4 days a week and the security gate is usuall lifted between 4 and 5 am every morning. SLOSS stated their are sometimes cones also blocking the gate, but not always, this morning was a busy morning and cones were not in front of the gate. After speaking with SLOSS I spoke with the operator, SHIELDS David, who stated he comes to the gym the same time every morning and the security gate is usually up, but when the gate is down it is difficult to see. He also had another member of the gym come talk to me and stated that the gate is difficult to see when it is in the down position. SHIELDS stated this morning he did not realize the gate was down and drove into it causing the damage. Upon my arrival I observed the entrance of the parking garage to be very well lit and the gate to be obvious.

Operator of MV1, Emily Stoller, states she was operating MV1 in the parking lot at 200 Boylston Street when her friend, Alvin Day, asked her if he could drive MV1, which she gave him permission to do and was under the impression Mr. Day had a valid driver's license. It should be noted this area is regularly used to get to commercial businesses and has the right of access to the public. Ms. Stoller states Mr. Day was only driving about 15mph when he lost control of the vehicle and crashed head on into a light pole. MV1 sustained heavy front end damage and was towed from the scene by Tody's towing. Mr. Day states he does not have a valid driver's license and asked Ms. Stoller if he could drive MV1. Mr. Day states he was "goofing off" driving around the parking lot at approximately 15mph when he "cut the steering wheel too much", lost control of the vehicle and crashed MV1 head on into the light pole causing the driver's side airbag to deploy. Mr. Day states he lost consciousness for a brief moment where he was assisted out of the vehicle by the rear passenger, Alex Kakooza. Mr. Day was evaluated by the medics and transported to Children's Hospital. Ms. Stoller and Mr. Kakooza were also evaluated by the medics where they obtained patient refusals. All three juveniles had an adult family member respond to the scene. Ms. Stoller's father, Gregory Stoller, Mr. Day's mother, Quonekuia Day and Mr. Kakooza's sister, Latifah Kakooza. Professional electrical contractors responded for the damaged light pole and mall security will be notifying the property owner.

On Monday 2/22/2021 at approx 0849hrs, while assigned to N491, I responded to 33 Austin St in Newton for a MVA hit and run. There I was met by the owner of MV1 who states that his car was parked in the Austin St lot while getting coffee at Starbucks. He states that a witness to the accident gave him their information. Witness statement: Schreiner states that she had just left Starbucks and heard a crash. When she looked into the Austin St parking lot she could see MV2 make contact with MV1. She said that MV2 then scrapped the side of MV1 with the front passenger side of the vehicle. When MV2 left the parking lot she said that she was able to read the MA plate of 1NTE29. Schreiner said that MV2 was white and looked like a Toyota Highlander operated by a white female w/ blonde hair in her 40's. It is unclear at this time who was operating MV2(MA# 1NTE29). Dracut PD was contacted about the registered owner and I was able to get a phone number. However, the number was to a business she hasn't worked at in five years. Traffic Bureau update (Officer Gaudet): On Monday, February 22, 2021 I was able to make contact with the registered owner of MV2, Ms. Melinda Russo (S29162725). Ms. Russo stated that she was operating her white 2019 Toyota Highlander (MA: 1NTE29) in the Austin Street municipal parking lot this morning. Ms. Russo stated she was backing out of a parking spot when she observed her passenger side mirror collapse in. Ms. Russo stated after she exited the parking spot she pulled her vehicle over and observed no damage to the passenger door mirror area. Ms. Russo stated she fixed her mirror and then left the area. Ms. Russo stated she did not think there would be damage to any other vehicle because she did not have damage on her vehicle. Ms. Russo was advised that if she believes she was involved in a crash, she should make every effort to contact the owner of the other vehicle. Ms. Russo was also advised she can contact the police department for assistance. Ms. Russo will be mailled Massachusetts Uniform Citation T1446762 for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care). Austin Street is a public way maintained by the City of Newton.

Operator Diego Colindres states he was stopped at the stop sign located on Central St westbound at Grove St. As he proceeded through the sign to drive onto Grove St, a female jogger ran out into the street from the sidewalk in front of 16 Grove. Colindres reports he did not see the jogger until his front bumper made contact with the jogger's leg, which in turn knocked her to the ground. The jogger then got up, said she was ok, and tried to continue on her way. Colindres asked her to stay on scene and wait for emergency personnel, to which she obliged. It should be noted that there are bushes located in front of 16 Grove St which substantially hinder the view of the sidewalk for vehicles approaching Grove from Central. Jogger Rachel Coffin confirmed that she was crossing Central St heading southbound on Grove St when she was hit by vehicle #1. Coffin was not sure if she stopped prior to crossing the street, and did not see the vehicle until contact was made. She does recall that the vehicle was traveling at a slow rate of speed when she was struck. Coffin's attire consisted of dark colored shorts, a navy t-shirt, and white sneakers. I observed a light red area on Coffin's outer left thigh consistent with the height of vehicle #1's front bumper. She was tended to by the medics and declined transport, repeatedly saying that she just wanted to get back to school (Lasell) and do her homework. Due to Coffin's uncooperativeness and multiple requests to leave the scene, pictures of her minor injury were not taken. There was no visible damage to vehicle #1. Traffic responded, however Coffin wished to leave prior to arrival.

The owner of MV#1 stated she had parked her vehicle in front of 50 Union St on Saturday 03/13 and discovered a hand written note on her windshield. The note was from a party who witnessed MV#2 struck MV#1. MV#1 sustained minor damages to its driver's side door (Lower panel). I spoke to the witness who stated on Saturday 03/13 at 14:30 he was sitting outside at 45 Union St (The Bar-A-Mor Restaurant) and observed MV#2 attempting to parallel park behind MV#1. The witness stated he heard a loud bang and observed MV#1 shook. The witness then observed 2 Asian parties (1 male party and 1 female party) exit MV#2 while carrying carpet/ rugs walking northbound on Langley Rd. The witness stated he was unsure if the male or the female party was operating MV#2. The witness described MV#2 as a black colored "BMW X5" SUV bearing New York registration GFS4413. The witness stated he was unsure what part of MV#2 made contact with MV#1 but believes it to be either its front passenger side tire area or rear passenger side tire area. I spoke to the operator of MV#2 and she stated she is the owner of a BMW X5 SUV bearing New York registration GFS4413 and was in Newton Massachusetts on Saturday 03/13 delivering some carpet/ rugs to a nearby business. The operator of MV#2 stated she was unaware she had made contact with any vehicles. The operator of MV#2 was forthcoming with her information and provided her driver's license number along with her insurance provider. Based on the statements made to me, I will not pursue this accident further. The operator of MV#2 was provided with this accident report number and advised to contact her insurance provider.

On March 29, 2021 at approximately 17:40 hours I responded to 33 Boylston St, Juice Pressed, for a report of a single vehicle crash into a building. Upon arrival, I observed a black Mercedes SUV (MV1) parked in front of Juice Pressed with glass on the hood of the vehicle. The window pane store front and right door to the "Juice Press" storefront were shattered and broken. The operator of MV1 said that she was stopped in the parking space in front of Juice pressed ordering her drinks via the mobile app for pickup when she thought the vehicle was in park she took her foot off of the brake and accidentally hit the gas peddle. The vehicle crashed into the front of the store traveling about 2-3 feet into the building, breaking the front glass and door. No patrons were within striking distance of glass or the vehicle. Two employees were inside the business and said they were 15-20ft away from where the vehicle made contact. Newton Fire were on scene, management for "the Street", and "Juice Pressed" were notified and responded and arranged for the storefront to be boarded up. Management and vehicle owners were given the accident report number and information on how to obtain the report.

On Wednesday April 7th, 2021 I responded to 309 Crafts Street for a report of a motor vehicle accident involving a City of Newton contracted school bus (mini-van). At the time of the accident the weather was cloudy and the road surface was dry. Crafts Street is operated and maintained by the City of Newton. Upon arrival I spoke with the operator of the school bus identified as Sybil Berry (S93963801). She stated she had just turned left from Walnut Street onto Crafts Street and was traveling eastbound when a parked vehicle on Crafts Street pulled into the travel lane at the same time as she was traveling straight, and made contact with her vehicle. Sybil's vehicle sustained minor passenger side panel damage. Photographs of the damage were taken and submitted to the IT Bureau. Sybil stated the operator of the motor vehicle couldn't wait for police to respond but he left his information. The operator of the other vehicle involved was identified as Jun Nakmoo. I was able to speak with Jun but there was a strong language barrier. Jun stated he was parked on Crafts street and did not see any vehicles coming his way so he pulled into traffic. Jun stated the mini van appeared out of nowhere and sideswiped his car. Jun reported minor damage to his drivers side panel. Both vehicles were able to drive safely from the scene. The school bus had two children passengers in the vehicle at the time of the accident. The children were identified as John Castellano (07/31/09) and Delsi Castellano (06/26/11). Both students were not injured and were picked up by their mother Adria Castellano (01/05/81). The children both attend Bowen Elementary School and were being transported to Waltham.

On Friday April 9, 2021 at approx. 1343 hours I responded to 281 Needham Street for a report of a hit/run involving two vehicles. Upon arrival I met with oper of V1. She stated she pulled into the parking lot of 281 Needham Street, she observed a open parking spot that was next to a blue van that was parked in the handicap spot next to the open spot. She stated she was backing into a parking spot when V2 ( the blue van) all of sudden backed into her vehicle on the pass side rear. She further stated the oper of V2 exited his vehicle started yelling at her and drove off. She described the man of being in his 60's. Witness stated he did not observe the crash but wrote down the registration plate of the blue van which was driving away after the crash which was MA reg. 7WH752. I queried the registration which came back to a 2013 Ford van color blue registered to K.LEVANA of 110 Woodcliff Rd Brookline. Dispatch attempted to leave messages. Dispatch was also able to find another party identified as MA lic # S13435214 Michael Hasson d.o.b. 9-27-49 of same address. I ran license to obtain R1 photo through my computer and the oper of V1 stated for sure that was the driver. Other attempts were made to contact Mr. Hasson and at approx. 1530 hours he came into NPD headquarters to make a statement. He stated he was parked in the handicap spot, he backed up and did not observe V1 until impact. He stated after impact he exited his van and an argument began with oper of V1. He stated he has a medical condition and had to leave to use the bathroom. I advised there are several businesses and he just left one that had a bathroom to use. He stated he was nervous but had to go the bathroom right away and left to go to another place to use the bathroom. I asked him if he got any medical treatment after the crash, he stated no. I asked him if he needed medical attention, he stated no. He also stated there was not any damage to his van. I asked him if he had a HP placard since he was parked in a HP spot, he stated he did not have a HP placard. I advised him of the violation for a HP. I advised at this time of a verbal warning. All parties were advised of process.

This accident was reported approx. 8 hours later due to reporting party not familiar with laws in the U.S. Bicyclist #1 doesn't speak English well and needed her husband (Siarhei Byk) to translate. Bicyclist #1 stated on 05/17/2020 at approx. 1600 hrs., she was travelling southbound on Chestnut Street on the sidewalk. While crossing Anita Cir, MV#1 approached the intersection and collided into her. She was thrown onto the hood of MV#1 as a result of the collision. The OP. of MV#1 helped her into his car and drove her home. The OP. of MV#1 only gave her a business card and nothing else. The husband of Bicyclist #1 came home later and drove Bicyclist #1 to the hospital to be checked out. The husband of Bicyclist #1 called the OP. of MV#1 to get some car insurance information and was denied. The OP. of MV#1 stated he felt uncomfortable giving that information and they would talk the next day about it. The husband of Bicyclist #1 felt this was strange and decided to report it to the police after getting out of the hospital. Bicyclist #1 was wearing black pants and a blue shirt. She did not have a helmet on and didn't have any reflective clothing on. She sustained bruising to both legs and didn't get a concussion. Her bicycle gears and her wheel was damaged from the accident and pictures were taken by me. The brand of the bicycle is Infinity Hybrid and it cost approx. $350.00 USD when new. Bicyclist #1 describes the OP. of MV#1 as a white male around 50's - 60's years of age. She describes the vehicle that struck her as a red SUV. The OP. of MV#1 gave a business card with the name Ruth Robart out of 28 Richardson Rd., Newton 617-818-8660. Further investigation revealed a Ralph Robart out of that address with a similar age. Due to the time of night, the investigation would be postponed to daylight hours. At approx. 1550 hrs., I made contact with the OP. of MV#1. He stated he was stopped on Anita Cir., waiting to make a right turn onto Chestnut Street. When there was no more oncoming traffic to his left, he looked to the right and saw it was clear as well and started to accelerate. That's when Bicyclist #1 suddenly rode out from the sidewalk and they collided together. There's a 6 foot wall from a residence that blocked his vision to see further down the sidewalk. The OP. of MV#1 stated there was no damage to his vehicle.

On 06/14/21 at approximately 12:00 hours, Sgt. Wade received an email from Nina Wang (D.P.W. Transportation Traffic Superintendent) that a city utility trailer with a signboard attached to it was struck by another motor vehicle at 275 Brookline Street sometime over the weekend between 06/11/21 and 06/14/21. A photo was sent over in the email of the utility trailer and signboard knocked over on it's side over the sidewalk. The utility trailer is 2020 Wanco color orange bearing MA Official registration # M9994A. The trailer was parked in the bicycle lane of the northbound travel lane next to 275 Brookline Street. The signboard was there in an attempt to get motorists to drive the speed limit due to the Memorial Spalding School located at 250 Brookline Street. On 06/14/21 at approximately 12:00 hours, I responded to Brookline Street to photograph the trailer and possibly process the scene. The trailer could not be located. I learned later that the trailer had already been towed by Tody's towing and brought to the City D.P.W. yard located at 110 Crafts Street. On 06/15/21 at approximately 09:00 hours, I went over to 110 Crafts Street and was told that the trailer/signboard was in the garage. The trailer was already being repaired to be placed back in to service. It had sustained damage to the two rear brackets where the jacks are located and the license plate bracket and registration plate were bent. The signboard message frame board is no longer aligned properly as a result of the accident but reportedly still functions. After, I then responded back to 275 Brookline Street and took digital photos of the scene. I observed in the bike lane at the accident scene two small pieces of lens covers to a vehicle with no identifying marks. I also did a canvass for possible surveillance cameras in the area. There was no response at number 269 and 275 Brookline Street but I left a note for them to call me. 283 Brookline Street has no surveillance cameras. Memorial Spalding School is across the street from the accident and I observed a camera on the side of the building which faces the staff parking lot on to Brookline Street. I spoke to a staff member Jesse Brackett at the school. He called David Stickney (Superintendent of Schools) and notified him of the accident. Mr. Stickney was provided with my email and will send over any possible footage if he has it. At approximately 12:45 hours, I spoke to Mike Sylvester who resides at 275 Brookline Street. He stated that he had surveillance footage and emailed it over to me. I viewed the footage which is a camera that views his approx. 75 foot driveway leading out to Brookline Street. There is a fence and bushes behind where the signboard is located which limits it's view. The video shows the accident took place on 06/11/21 at 18:48 hours. At this time, you can see the trailer is struck and slowly falls over on it's side. Immediately after a white colored four door sedan continues to slowly drive past the scene northbound on Brookline Street in the bicycle lane without stopping. Two other vehicles behind the white sedan drive around to the left of the white vehicle at this point. In the footage, you can not view the registration number, operator details, or the type of sedan from the footage. On 06/16/21 at 09:40 hours, I spoke to David Stickney who called the Traffic Bureau. He stated that he had footage of the accident but it did not reveal any other details besides the same white sedan striking the trailer.

On Wednesday, June 16, 2021 while assigned to Traffic unit N525, I responded to the front entrance of the Brown Middle School located at 125 Meadowbrook Road in Newton for a single vehicle crash involving a City of Newton contracted school bus. It was also reported by Newton Dispatch that one of the occupants on the bus had suffered an arm injury as a result of the crash. Meadowbrook Road is a public way maintained by the City of Newton. The road surface at the time of the crash was dry. The weather at the time of the crash was clear and sunny. When I arrived, I was updated by Officer Iarossi that the crash involved a City of Newton contracted school bus transporting students from the Williams School (141 Grove Street, Newton). The students were participating in "Step Up" day and were leaving Brown to go back to Williams. Officer Iarossi stated a student on the bus was injured as a result of the crash. Tegan Quirk was transported by Fallon Medics to Newton Wellesley Hospital with a right arm injury. The remaining students that were on the bus reported no injuries to staff at the time and were moved to another bus to be transported back to the Williams School. I asked the Brown staff that was assembled in front of the school to contact the Williams School and ask them to hold the children involved in the crash at the school once they arrived. I made this request so we could make sure there weren't any new injuries reported; and I wanted to inquire if any of the children witnessed the incident. The staff stated they would contact Williams and relay the request. I spoke with the operator of the school bus (MA BUN: BU44536), Mr. John Aubert (S48613114). Mr. Aubert stated he was operating bus #361 while transporting students from Brown Middle School to the Williams School. Mr. Aubert stated as he was preparing to leave Brown, he made an announcement to students on the bus to sit down. Mr. Aubert stated he got the okay to leave from one of the staff members on the bus. Mr. Aubert stated there was another bus parked in front of him prior to leaving. Mr. Aubert stated he began to reverse the bus to create distance from the bus in front of him, and then operated the bus forward to exit from his position on the right side of the road and merge into the travel lane to his left around the bus parked in front of him. Mr. Aubert stated as he merged onto the roadway, he heard screaming from the students on the bus and the rear passenger side of the bus made contact with a brick support column in front of the school positioned along the sidewalks edge next to the roadway. Mr. Aubert stated he exited the bus and observed a student's arm pinned on the rear passenger side of the bus between the side of the bus and the brick column. Mr. Aubert stated he got back in the bus and reversed it until the students arm was free. Mr. Aubert stated at this time, staff helped the injured student. The staff there helped facilitate transportation for the remaining students back to the Williams School. Mr. Aubert reported no injuries, but was concerned for the welfare of the student. After speaking with Mr. Aubert, I observed damage to the school bus and the brick support column as a result of the crash. I observed fresh damage (scrapes) to the rear passenger side of the school bus between the last passenger side window and the back of the bus. I also observed fresh damage to the brick support column. The column is a support to an overhead covering that runs from the sidewalk to the front entrance of the school. There was fresh paint transfer from the bus and a crack in the column. Pictures were taken of the damage and submitted to the IT Bureau. The travel lane where the crash took place is a one way access road that runs from Meadowbrook Road to the front of the school, and exits onto a parking lot between the Brown Middle School and the Oak Hill Middle School. I responded to the Williams School to follow up with staff there regarding this crash. All of the students that were involved in the crash had been released prior to my arrival. I spoke with the Principal of the Williams School, Ms. Ayesha Farag, Ms. Michal Shapiro (Social Worker), and Ms. Katie Costello (School Nurse) about the students on the bus. They stated no injuries were reported to them, and they notified the parents of the involved parties about the crash. They also provided me with contact information for all of the students on the bus at the time of the crash. I spoke with the three teachers that were on the bus at the time of the crash, Ms. Kimberly Andersen, Ms. Tess McLean, and Ms. Katherine O'Leary. All three teachers stated prior to the bus leaving Brown Middle School they accounted for all of the students and were ready to leave to travel back to the Williams School. One of the teachers recalled hearing the bus driver make an announcement to the bus that they were about to leave, but could not recall the exact wording of the announcement. The teachers stated as the bus began to move, they heard screaming from the rear of the bus, looked back, and saw a student's arm stuck outside the window. After realizing the student was injured, they assisted with helping to free the student from his pinned position. They helped the injured student until EMS arrived on scene. They assisted the remaining students by directing them away from the crash and onto another bus for transportation back to Williams. The three teachers reported no injuries as a result of the crash. I responded to the Newton Wellesley Hospital Pediatric Emergency Room to check on the status of the injured student, Tegan Quirk. I spoke with Tegan and his mother, Kathryn Quirk in his room. While attempting to talk to Tegan and his mother, Tegan became very upset and did not want to talk about the specifics of the crash. Out of courtesy for Tegan, I exited the room and spoke with his mother outside the room. I explained to Ms. Quirk the events of the crash, and how her son's arm became injured. I asked Ms. Quirk if she could assist me with faciliating a conversation with Tegan about the events of the crash. I explained to her I understood how traumatic the events of the crash were for her son, but as part of my investigation I would need to find out what led to Tegan's arm being outside the bus window at the time of the crash. While speaking to his mother, Tegan explained that he was waving to a friend with his arm out of the bus window when the bus started to move. Tegan stated his arm hit the column, and became stuck between the bus and the column. Tegan stated he was screaming for the bus to move and that it took forever for the bus to move so he could free his arm. Tegan stated the bus driver gave no warning to the students that the bus was going to start moving. The staff at Newton Wellesley Hospital stated Tegan has a right arm injury. The severity of the injury is unknown at this time due to further examination that needs to take place. I advised Ms. Quirk about the conversations I had with the other parties involved. I spoke with Mr. David Stickney (Head of Facilities) from the Newton Public Schools to try and obtain footage of the crash. Mr. Stickney stated unfortunately there is not footage of the crash from the Brown Middle School or Oak Hill Middle School surveillance cameras. I advised Ms. Regina Moody (Newton Public Schools Transportation Director) about the crash. Ms. Moody was able to show me footage of the incident from the camera inside the bus. In the footage, the bus driver can be heard announcing to the students prior to the bus moving, "Guys, guys, get off the seat and sit correctly". One of the teachers can be seen doing a count of the students. The teacher is heard saying to the student, "Everyone needs to be in a seat and sitting at 3 and 2 and 1 and thank you." Student are observed still standing up. The teacher is then heard saying, "sit down sit down." After the teacher passes the students in the back of the bus, she says, "We are good." to the front of the bus. Students are then observed standing after she passes. As the bus begins to back up, students can be observed standing in the rear of the bus and not sitting down on their seats. As the bus moves forward, a student (Tegan Quirk) can be seen standing on his seat and extending his right arm out of the last window on the passenger side of the bus. Screaming can then be heard as his arm makes contact with a support column on the passenger side of the bus. The bus is stopped, the driver exits the bus to asses what happend, gets back on the bus, and reverses the bus a short distance. The injured students right arm is released and he brings his arm back into the bus through the window. The amount of time between the students arm being pinned against the column to when it is released is approximately 40 seconds. After speaking with all parties involved and reviewing the footage from inside the bus, it appears that Bus #361 was pulling out of it's parked position in front of the Brown Middle School to enter the roadway and return students to the Williams School. A student that was sitting in the last row on the passenger side of the bus extended his right arm out of a window next to his seat to say goodbye to a friend. While his right arm was extended out of the rear passenger side window of the bus , the bus collided with a brick support column affixed to the sidewalk in front of the school. When the bus made contact with the column, the students right arm made contact with a brick support column and became pinned between the column and the passegner side of the bus. After the students arm was freed from it's pinned position, the student was able to bring his arm back into the school bus and was transported to Newton Wellesley Hospital for a right arm injury.

HE STATES HE IS NOT LICENSED BUT SINCE HIS DAD WAS BUSY AND THE OPERATOR OF VEHICLE #1 WANTED TO GET OUT HE FIGURED HE WOULD DO HIS DAD A SOLID AND MOVE THE CAR FOR HIM. HE STATES HE PLACED THE CAR IN REVERSE AND WHILE BACKING WENT TO QUICKLY AND WAS UNABLE TO AVOID HITTING VEHICLE #1. THERE WAS NO DAMAGE TO VEHICLE #2. On July 5th 2021 while assigned to NPHQ front desk, Jaden Chin DOB 4/5/05 came in with his father Michael Chin to report a past auto vs bicyclist accident. Jaden stated the accident occurred on June 19th at the intersection of Beacon St and Glen Ave. When I inquired as to the reason this was being reported several weeks after the incident itself, Jaden and his father originally told me that they thought it was the motorist's responsibility to report it. After a few minutes of conversation it was learned that the Chin family had recently been contacted by the insurance company of the involved driver, allegedly due to exterior damage on the vehicle exceeding cost expectations. I was then told that a verbal agreement had previously been made by all parties to handle any incurred costs (whether they be medical bills for Jaden or auto body bills for the vehicle) without involving insurance. The recent call from the insurance company prompted Michael and Jaden Chin to come in to file a report. I then advised them that although I will document a statement from Jaden, the circumstances no longer provide for a standard police crash investigation. Jaden stated he was riding his bicycle on the sidewalk of Beacon St heading westbound towards Newton Center, when he and a black Mercedes collided in a "t-bone" manner as the Mercedes was pulling out of Glen Ave to turn onto Beacon. Jaden stated he hit a passenger side door of the car when he was crossing Glen Ave, dropping to the ground on his bike after impact. He added that there were bushes blocking his view of Glen Ave and is not sure if he stopped before entering the roadway. Injuries reported are jaw pain and minor bruising. The driver left his information with Jaden and is identifed as Joowon Park DOB 6/15/88. He has an active license and CJIS has him listed as leasing MA reg 9EB432, a black Mercedes GLC. I attempted to call Park at the number provided to me by Jaden Chin with negative results. All streets mentioned are public ways in the City of Newton. The Chins were advised that a report will be on file documenting Jaden's statement, but that in the future they need to call police immediately after an incident like this occurs.

Homeowners at #289 Cherry St., identified as Midge Connolly and Walter Devine called NPD to report that a delivery truck had driven on their property causing damage. On arrival I observed significant damage to the property at #289 Cherry St along the Upham St side of the property. Damage included the lawn, bushes between the driveway and Upham St, a tree and barrels. I then spoke with the operator of MV#1 who stated that while trying to back his truck into the driveway at #297 Cherry St, which is actually located on Upham St, he backed over the property at #289 Cherry St causing the damage listed above. Operator stated he was driving west on Charlesbank Rd and was attempting to make a left turn onto St James St when he struck the concrete island barrier that divides east and west traffic to Charelesbank Rd. Operator further stated due to his windshield fogging up from the weather/heavy rain he didn't see the island barrier. This vehicle I found out after the operator left with a LYFT driver was a short time rental from KYLAS Auto body, #819 MAIN St Brockton Ma. I contacted that business and spoke with the receptionist, named Ariann. I informed her about the accident and that I didn't identify the operator for my report. She was going to attempt to contact the person who rented the vehicle and get back to me. The person that rented the vehicle was a woman out of Brockton. KYLAS Auto was going to contact her to identify the person who she allowed to drive the vehicle this morning. The woman identified by KYLA'S Auto body as the person who rented the involved vehicle was a Maria Amado, 234 Green St, Brockton Ma. 02301. Kyla's Auto body will be going after her insurance carrier for the damage to their vehicle.

On 07/28/21 at 13:39 hours, I (Traffic Unit # 523) responded to 10 Cummings Road for a past hit and run motor vehicle bicyclist crash which occurred approximately twenty minutes prior. Dispatch updated units that the accident occurred at the intersection of Watertown and Walnut Streets and that a white colored SUV left the scene after striking the bicyclist. The white SUV was last seen heading southbound on Walnut Street towards Washington Street. The bicyclist then walked back home to 10 Cummings Road and told her mother about the incident. The mother then notified NPD. Watertown Street and Walnut Street are both public ways maintained by the City of Newton. It is a four way intersection with crosswalks on every side. There are no turn on red signs on every corner of the intersection. The weather at the time was partly cloudy and sunny and the road conditions were dry. On arrival at 10 Cummings Road, I spoke to the mother identified as Vanessa Cruz and her daughter, Angelique Cruz, who was involved in the crash. Fallon Ambulance was already on scene and a patient refusal was signed as Angelique did not have any injuries. I asked Angelique what had occurred and she stated that she left FA Day Middle School at approximately 13:00 hours. It is estimated that the crash took place between 13:10 and 13:20 hours. She was then bicycling down to the intersection of Walnut & Watertown Streets. She says that she pushed the walk signal button to cross the street at the crosswalk to continue southbound on Walnut Street towards Lowell Avenue. Once the cross signal light activated, she bicycled through the crosswalk. At the tail end of the crosswalk, her rear bicycle tire on the right side was struck by a white SUV making a right turn on to Walnut Street from Watertown Street. She was then knocked to the ground where she sustained a scrape mark to her left knee on her sweat pants. The white SUV continued southbound on to Walnut Street without stopping. She feels that the driver might of misjudged taking the right turn and never realized that she struck her. Angelique says she only saw a female operator in the SUV and could not determine the race or age. She also did not observe the registration on the vehicle. She further stated that a female assisted her up from the ground. Once she said she was fine she then proceeded home. No one else phoned 911 regarding the incident. At this point, I asked to see the bicycle which was stored away. The bicycle is a Schwinn Sidewinder 21 speed color black with lime green block style writing for the brand and model on it. Both tires have white reflectors attached to them. I observed no damage to the bicycle. At the time of the crash, Angelique stated she was wearing purple sweat pants, blue sweater, and no bicycle helmet. She was also carrying a black backpack attached to her rear shoulders. I took digital photos of Angelique along with her Schwinn bicycle. After, I went over to the intersection of Watertown and Walnut Streets and took photos of the scene. A canvass was also conducted that produced negative results. I observed no surveillance cameras on the buildings or residences that are near the intersection. At 109 Walnut Street, I spoke to several tenants in the six unit apartment complex and they have no security cameras. 583 Watertown Street has no cameras along with 592 Watertown Street. I also checked near the intersection of Walnut and Washington Streets as the white SUV was last seen heading in that direction. The Trio apartment complex along with other businesses are lined against both streets and have outside security cameras. I spoke to the leasing office and they stated that the cameras do not provide any coverage of the street area.

Centre Street and Church Street are public ways in the City of Newton. The OP. of MV#1 states he was travelling southbound on Centre Street, proceeding through a green light when a vehicle suddenly came out of a side street and collided with him. That vehicle struck the curb and continued down Centre Street without stopping. The OP. of MV#2 states he isn't from around here and was trying to get home to Worcester. He was driving down an unknown roadway when a vehicle suddenly came into his lane and collided with him. There weren't any shoulders on the roadway so he couldn't pull over right away and kept driving until he found a safe area. Witness#1 (Mr. TING) stated he was driving behind MV#1 and they had the green light on Centre Street. MV#2 came out of Church Street and collided into MV#1, then crashed into the curb and continued driving down Centre Street without stopping. Witness#2 (Mr. O'CONNELL) stated he was at the front porch of 150 Church Street when he heard two loud crashes. He observed a motor vehicle accident and a black vehicle kept driving without stopping and the silver vehicle was stopped at the intersection. Witness#3 (Mr. SPENSIERI) stated he was at the front porch of 150 Church Street when he heard two loud crashes before looking over. He observed a motor vehicle accident where another vehicle kept driving away from the scene. He ran after the vehicle that wasn't stopping (MV#2) and was able to follow along because MV#2 was going slowly with blown tires. He followed MV#2 down Centre Street towards Washington St. MV#2 took a right at the intersection and continued down Washington Street before making another right turn onto Park Street. MV#2 finally stopped at Elmwood Street by Park Street where Mr. SPENSIERI phoned it in. The OP. of MV#1 signed a patient refusal with medics. The OP. of MV#2 complained to Sgt. Walton of a head injury and was transported to St. Elizabeth's Hospital. There was a MBTA bus sign knocked over at the accident location. There are multiple parking spots on Centre Street and on Vernon Street that the OP. of MV#2 could have pulled over to be safely out of traffic but he did not. He drove for approx. 0.3 miles on two flat tires before pulling over at Elmwood Street. The OP. of MV#2 was advised that he will be receiving a citation in the mail for his motor vehicle violations.

On Wednesday, September 1, 2021, while assigned to marked unit N525, I responded to the area of 1488 Washington Street, Newton for a report of a motor vehicle crash involving a school bus. The weather at the time of the crash was cloudy was light rain. The road surface was wet. Washington Street is a public way maintained by the City of Newton. The operator of MV1, Fanes Nerestant (S47503048) stated he was operating a City of Boston owned 2012 Chevrolet School Bus (MA SBN: SB37810) west on Washington Street towards the Learning Prep. Mr. Nerestant stated he slowed his vehicle down and put on his right direction to take a right turn into the parking lot at 1488 Washington Street to pick up students at the Learning Prep School. Mr. Nerestant stated he then felt impact from the back of the bus and heard a "boom". Mr. Nerestant was transported to Newton Wellesley Hospital for neck and back pain. I observed minor damage to the rear driver side bumper/fender area. There were no other occupants on the bus at the time of the crash. The operator of MV2, Mr. Allen Simon (S44212695) stated he was operating his 2018 Nissan Altima (MA: CS12BM) west on Washington Street near 1488 Washington Street. Mr. Simon stated the school bus in front of him in his travel lane abruptly stopped to make a turn and he was unable to avoid crashing into the vehicle. MV2's front passenger side front end sustained heavy damage as a result of the collison. Mr. Simon reported no injuries. MV2 was removed from the roadway by Tody's towing. Mr. Simon stated his vehicle had a camera in it at the time of the crash and Mr. Simon stated he would forward the footage of the crash to me.

On September 2, 2021, I responded to the front desk of Newton Police Headquarters to speak with Charles Rankin who wanted to report a past hit and run. Mr. Rankin stated his vehicle (vehicle 1) was parked unoccupied by the curb at 70 Orchard Ave ( Public way in the City of Newton) on Tuesday, August 31. Later that day, he discovered damage to his drivers side mirror. He stated that a note was left behind stating that Massachusetts registration: V91699 struck his vehicle and left the area. He does not know who left the note due to no contact information being left behind. Mr. Rankin showed me a picture of the damage and it appears vehicle 2 side swiped vehicle 1 causing significant damage to the mirror. Mr. Rankin did research and discovered that the plate is registered to JCAC Enterprise out of Milford and provided me with a phone number. It should be noted that when I queried the plate it comes back to that company as well. I will attempt to follow up with the company during business hours on my next tour of duty. On September 3, I called JCAC Enterprise and spoke with an employee who stated they do not have any information on someone driving a truck in the area. They also stated that Mr. Rankin called them and they are going to pay for damages.

Mv#1 was parked unoccupied opposite #8 Burrage Rd (N/B). #1 was sideswiped by an unknown Mv that left white paint transfer damage on #1's driver's side front end side and also struck the driver's side mirror all causing moderate damage. I spoke to the resident at 8 Burrage Rd. The resident stated a visitor in his house, Jonathan Kaplan, witnessed the accident. The resident stated Kaplan told him he observed a white sedan bearing MA Reg CIAF31 (Mv#2) attempting to parallel park between #1 and another Mv. During the attempt, #2 was observed sideswiping #1. Kaplan further stated a male operator got out of #2, left a note on #1 and drove away. Mv#1 owner/operator Ms Joan Beer stated no note was ever found. Later from HQ, I called the witness phone number provided (617-455-2557) by the resident of 8 Burrage Rd. I called that number, received VM and left a message. A query of #2 registration comes back on a '18 white Lexus G350 to a Mr Barry Benjamin of 79 Florence St #510S Newton. I received information Mr Benjamin's phone number was 339-223-0079. This number when called was not Mr Benjamin's. Later from HQ, I spoke to Ms Beer and provided her with the witness plate number observed as the paint color is a match to the damage sustained on her Mv. She was advised insurance company will investigate further based on current information received. Later, the witness Mr Kaplan returned my phone call and provided the following statement: Kaplan stated he was in his driveway outside directly across the street from where the accident was taking place. Kaplan stated the accident was very obvious as damage was unfolding while Mr Benjamin was attempting to squeeze between Mv#1 and another vehicle. Kaplan stated after realizing he (Mr Benjamin) could not fit he started to drive away. Kaplan stated it drew the attention of another witness (unknown) standing on the street when both walked up to Mr Benjamin's vehicle and asked him if he was going to leave a note for the damaged vehicles. Benjamin refused. Kaplan described Benjamin's overall demeanor as "rude." Kaplan stated he told Benjamin he has cameras capturing the incident. Not until then, Benjamin got out of his car and left a note on only the other vehicle reported to be minor license plate damage. Kaplan stated Benjamin never left a note on Mv#1, the vehicle with obvious moderate damage. Kaplan stated he actually viewed the note left on the other vehicle and said it was illegible. Kaplan told Benjamin this and Benjamin yelled to him "mind your own business" and drove away. Kaplan stated his ring doorbell camera was not positioned to capture any of the incident. Traffic Bureau update (Officer Gaudet): On September 8, 2021 I was able to make contact with the operator of MV2, Mr. Barry Benjamin (S42076881). Mr. Benjamin stated he was operating his 2018 Lexus G350 (MA: CIAF31) at the time of the crash. Mr. Benjamin stated he was backing his vehicle into a parking spot and the space ended up being too small. Mr. Benjamin stated the rear of his vehicle crashed into the front bumper area of MV1, knocking off it's license plate. Mr. Benjamin stated he got out of his vehicle and put the plate back on MV1. Mr. Benjamin stated he left a note in the vehicle with his contact information and left the area. I asked Mr. Benjamin if he was aware of making contact with another vehicle in the area. Mr. Benjamin stated he did not make contact with any other vehicle. Mr. Benjamin stated there was no damage to his vehicle and stated he would e-mail me pictures of his vehicle and pictures of MV1 that he took on scene. Those pictures were forwarded to the IT Bureau to be added to this report. Mr. Benjamin also stated in his e-mail, "My wife informed me that the homeowner who contacted the police called my wifes attention to a car up the street. The car was on the opposite side near a dirt road that had been side swiped earlier. On September 13, 2021 at approximately 17:32 hours I responded to the parking lot of Bloomingdales, 225 Boylston St for a report of a hit and run. I spoke with the operator of MV1 who said that she was traveling northbound on the parking lot roadway off of Boylston St by Bloomingdales Men's Store. MV1 was stopped at the stop sign when she observed what she believes to be a newer Mercedes SUV turn right onto the roadway and crash into the rear of her vehicle. MV1 exited her vehicle to try and get the driver's attention but the operator continued driving away. The operator of MV1 said that an MBTA bus was at the bus stop and saw the incident but was gone on my arrival. I was unable to locate any cameras in the area facing the crash location.

On Wednesday 9/15/2021 at approx 1045hrs, while assigned to the cell block, I took a past MVA report at the front desk. There I was met by Michael Krongel who states that at approx 1005hrs this morning his parked vehicle(MA reg1XJV53) was hit by another car at the Newton Wellesley Hospital parking garage. A witness stood by and gave Krongel their info along with a license plate number for the vehicle that struck his. The rear driver side of his vehicle was damaged. The second vehicle came back to a 2020 Ford Fusion MA reg 1LZR85 owned by Breanna Wasson DOB 4/23/1993. See witness statement. Newton Dispatch was able to find a number for Wasson on record. I called the number and left a message on its voicemail. It it not known at this time if Wasson was operating MV2. Traffic Bureau update (Officer Gaudet): On Thursday, September 16, 2021, I attempted to make contact with the registered owner of MV2, Ms. Breann Wasson, with a negative result. A message was left for her to contact me. A "Hit and Run" inquiry was mailed to Ms. Breann's address listed with the registry. On Thurday, September 23, 2021, I spoke with Ms. Wasson via telephone. Ms. Wasson acknowledged she was involved in the crash and had to leave with out immediately exchanging information because she was late for an appointment. Ms. Wasson stated when she returned to her vehicle, Mr. Krongel had left his business card on her winshield. Ms. Wasson stated she contacted Mr. Krongel to exchange information but he was not interested in speaking with her. Ms. Wasson stated she contacted her insurance company and reported the crash to them. Ms. Wasson did not provide information about any damage to her vehicle. Due to Ms. Wasson admitting fault for the crash and reporting the crash to her insurance company, no further action will be taken.
