Officer A. Kane (MV#2) was parked S/B on Cherry st. @ Pleasant St. processing a past MVA accident. While MV#2 was parked, the vehicle was sideswiped by MV#1 which was travelling S/B on Cherry St. Road conditions were slippery and it was snowing at the time of the crash. MV#2 suffered damage to Drivers side door and drivers front quarter panel. MV#1 suffered damage to front passenger side quarter panel. No injuries reported.

On 1/19/19 I responded to 25 Normandy Rd for a report of a past hit and run. I spoke with the Operator of MV#1 who stated the accident had taken place in front of Tom's Pizza, 423 Lexington St, at approximately 1830 hrs. Operator of MV#1 stated she parked her vehicle in a parking spot directly in front of the restaurant and went inside. When she returned approximately 5 minutes later she realized her vehicle had been rear ended. Two witnesses provided her with information about the accident and the plate number. (See witness narratives.) MV#1 suffered moderate damage to the rear and was driven from the scene. Waltham PD responded to the address listed for MV#2 and were not able to locate the vehicle. At approximately 2000 hrs, I was contacted at the police station by the owner of MV#2's wife. After further investigation (see incident report #19002425) it was determined that the operator of MV#2 at the time of the accident was the vehicle owner's brother. I spoke with Operator of MV#2 on the phone and he stated that the road was icy and slippery due to snow and he crashed into the back of MV#1. He stated he was nervous and thought he had only hit the sidewalk curb. Operator of MV#2 was issued citation T1440626 by mail for 90/24C Leaving the Scene of Property Damage, 90/10A Unlicensed Operation and 90/24 Use of MV Without Authority. He stated the vehicle suffered minor damage to the front.

Vehicle #1 stated she was traveling south bound on Grant Ave. Vehicle #1 stated that in the area where Grant Ave. bends to the left the roadway is dark and she had trouble seeing the road. Vehicle #1 stated at the last second she saw the road bending left and attempted to turn but skid on the snow and struck a small boulder in the front lawn of 173 Grant Ave. moving the rock a few feet. Vehicle #1 then skidded and came to a stop in the middle of the lawn of 173 Grant Ave. Vehicle # 1 then pulled off the lawn and to the side of the road. The owner of the property of 173 Grant Ave. was also present and was notified of the accident. Vehicle #1 had moderate damage to the front of the vehicle but was still able to be driven and was driven home. Both the operator and passenger in Vehicle #1 stated they were uninjured in the accident and refused any medical treatment.

MV1 (MA Reg: 63EP17, a 2000 Acura 32TL; operator: Carlos Standford) was traveling NORTH on Cherry St when a vehicle swirved into his lane head-on. In order to avoid MV3 he swirved into the other lane and side-swiped MV2. MV2 (MA Reg: 9JX388, a 2019 Subaru Outback; operator: James Cant) was traveling SOUTH on Cherry St when he had come to a complete stop, put his left turn signal on to show him turning left into his driveway at 176 Cherry St. He was waiting to make his turn until MV1 had driven past so it was safe to do so. As he was at a complete stop, MV1 collided with his rear driver side bumper in an effort to avoid a head on collision with MV3. MV3 (MA Reg: 93YE85; a 1997 Toyota Camry; operator Matthew Kuniholm) was traveling SOUTH on Cherry St when he had seen MV2 at a complete stop with a turn signal. He didn't give himself enough time to stop, began to slide as he was braking and went over the marked lanes into oncoming traffic in an effort to not collide with MV2. MV3 operator, Matthew Kuniholm, was issued in hand MA Uniform Citation T1440470 for Marked Lane Violation 89/9a. All vehicles were still in driveable condition. MV1 had damage to the front driveside wheel fender and driver door. MV2 had slight damage to rear driver side bumper. MV3 did not have damage to his vehicle. Weather was icy and there was an active snow fall during the time of the accident, which was 1815hrs.

Operator of MV1 stated he was traveling Eastbound on Commonwealth ave in his lane of travel when he observed MV2 parked on the right hand portion on Commonwealth ave. Operator of MV1 stated as he was approaching MV2 he attempted to slow down in an effort to avoid colliding with MV2, but stated he could not stop due to the slippery surfaces on the road and collided with MV2. MV2 was parked and unoccupied at the time of the accident. I spoke to Owner of MV2 who shorty arrived on scene after the accident who stated he parked in front of 676 Commonwealth ave for a while to shovel snow in his driveway. No injuries. MV1 was towed by AAA and MV2 was driven off by owner to his nearby residence. MV1 sustained heavy right front end damage. MV2 sustained heavy rear end damage.

On the above date/time I responded to 503 Watertown Street for report of a mva hit/run with property damage. Upon arrival I met with the owner of 503 Watertown Street Ms. Antonietta Tempesta she stated through he daughter in law Ms. Tiz Acuri at approx. 0830 hours she noticed fresh tire impressions on the roadway leading onto the sidewalk and into her fence that was damaged. She also stated the MA registration plate # 5DT664 was on the fence. I ran the the plate which came back to a 2003 Ford pickup color white to Kyle Butler of 84 Auburndale Ave Newton MA. Officer Boyle in unit 493 went to the residence and spoke with Kyle's uncle to call NPD. I advised Ms. Acuri of the process. Mr. Butler called NPD I spoke with him on the phone. He stated he was driving west on Watertown Street and due to the icy roadway his truck slid to the right onto the sidewalk and into what he thought was a snow bank. I gave all parties each others information and advised them. I gave the uncle Kyles reg plate.

On the above date/time I responded to 647 Washington Street for a report of a two vehicle crash. Upon arrival I met with the operator of vehicle two Mr. Weiner, he stated he was stopped in the Whole foods parking lot when vehicle one backed up into his vehicle. They exchanged information but the operator of vehicle one Ms. Adetola left prior to my arrival. I obtained her license information through Mr. Weiner and when I ran the reg plate on vehicle one it came back to Watertown Ford. I advised Mr. Weiner of the process and I will contact Watertown Ford for more contact information on Ms. Adetola to obtain a statement. Update:Jan. 21, 2019 On Jan. 21, 2019 I contacted Watertown Ford advised them of the crash and obtained a phone number for the operator of vehicle one. I spoke to her on the phone she stated the parking lot roadway was covered in snow/ice, she was backing out of the parking spot and vehicle two continued traveling and struck her vehicle. I advised her of the process.

On 1/21/19 I was working N498 when I responded to 17 Charles River Ter for a report of a Cab that had struck a wall. Upon arrival I met with Pema Jigmey and Miriam Jost. Jost stated that Pema who was driving for Metro Cab (or Photo Cab) picked her up at the airport and brought her home. Once at her home 17 Charles River Ter, she asked him to back into the driveway due to the snow and ice conditions. While backing up in the driveway of 17 Charles River Ter the vehicle struck the residencet causing damage. Pema stated the same thing but believed Jost was half to blame due to the fact she asked him to be dropped in the driveway. I informed Pema that he drove the vehicle into the house and so was at fault. The house only appeared to suffer minor cosmetic damages but could not be accurately examined. Pema and Jost exchanged information in the hopes of settling the damages without insurance. Both parties were satisfied and provided a report number. Both parties stated they were uninjured and refused medical attention.

On 01/22/19 I responded to a three vehicle crash on Needham St. in front on #241- Marshalls Plaza. The operator of veh #1 stated she was driving due West on Needham St. when she looked right at a car waiting to pull out of the parking lot. When she looked back to Needham St., the traffic was stopped and she hit her brakes and snow slid covered her windshield. Operator of veh #1 never saw the impact, but veh #1 struck stopped veh #2 which was pushed into stopped veh #3. Moderate damage to veh #1 and 2 but no damage at all to veh #3. Operator of veh #2 said she was going "2-3 MPH " when she was pushed into veh #3. Operator of veh #3 claimed she was dizzy from the accident and signed a patient refusal from Ambulance #1. Engine #7 was also on scene assisting. Operator of veh #1 was cited in hand Mass citation #T1441211 for Ch 90/ Sec 13- failure to remove snow from vehicle . All vehicles were driven away from the scene.

Operator 1 who was operating a marked police unit stated that while responding to an emergency with his Aux. lights and siren activated he was traveling Northbound on Hammond St. Operator 1 approached the traffic lights at Beacon street where he waited for a white SUV to clear from his path. When it was clear to go Operator 1 proceeded into the intersection where he could see vehicle 2 coming down Beacon St; however, he was unable to stop due the snow/ice on the roadway. Vehicle 1 and vehicle 2 collided in the intersection. Operator 2 stated that he was traveling Eastbound on Beacon St approaching a green light an Hammond St. Operator 2 proceeded through the green light and saw a white SUV pulling into the intersection from Hammond St going through a red light. Operator 2 began to stop and then witnessed vehicle 1 with its lights on. Operator 2 tried to stop ;however, do to the snow/ice on the roadway he was unable to stop. Vehicle 2 and vehicle 1 collided in the intersection. Both vehicles sustained minor damage and there were no injuries.

On Tuesday January 22nd 2019 at approximately 1615 hours I responded to a two car motor vehicle accident at the intersection of Beacon Street and Crescent Ave, both are public ways in the City of Newton. At the time of the accident the current weather conditions were clear and dry with the road conditions extremely icy and snow covered. Upon arrival the medics were evaluating the operator of MV1 for slight back pain. The operator of MV1 signed a patient refusal and stated she was headed westbound on Beacon Street and stopped at the intersection of Crescent Ave, attempting to turn left. The operator of MV1 stated she came to a complete stop and had her left turn signal on waiting to turn onto Crescent Ave. She states MV2 then rear ended her and she pulled over the side of the road to wait for police assistance. MV1 was completely disabled with major rear end damage. MV1 was towed by Todys. The operator of MV2 stated he was traveling behind MV1 and attempted to stop behind MV1 but due to the icy road conditions, MV2 failed to stop and collided with MV1. The operator of MV2 refused medical attention and was able to drive MV2 from the scene.

Leary was crossing Needham St southbound at the corner of the property between Stop and Shop and Jiffy Lube. She states that she was 1/4 of the way across the street when she was struck by MV1. The left side of her body(arm, hip) struck the right side of the MV. She fell to the ground. MV1 states that he was pulling out of the Stop and Shop driveway to turn right onto Needham St. He saw the pedestrian next to the snow bank on the right side of the driveway as he approached the end of the driveway. When he got to the end he did not see her anymore and assumed she had continued walking west on Needham St. He then turned right and felt the collision and then he saw her. He immediately pulled into the Jiffy Lube lot to check on her. I took pictures of the scene and the MV. They were TOT IT. After further investigation, on 2/7 the operator was cited for 90/14 failure to stop for pedestrian. I spoke with Kenneth via phone and advised him of the update. The citation was mailed to him. I am charging for 90/14 failure to stop for a pedestrian. On Wednesday February 6, the operator information of this report was ammended to show the correct operator information.

On 02/01/2019, while assigned to N494, I, Officer Conary, responded to Crafts St at North Street for a three car MVA. Operator of MV1 stated that she was sitting at the red light on Craft Street at North Street when she saw it turn green and then felt MV2 hit MV1 rear. Operator of MV2 stated that he was sitting at the red light at the intersection when he began to move when the light turned green and then MV3 hit MV2 rear. MV2 stated when MV3 hit him, he then hit MV1. Operator of MV3 stated that she was at the intersection when she began to slowly move with traffic, her foot slipped off the break, and she hit MV2. Operator of MV3 stated that her foot was wet from walking in the snow. There was snow on the ground but not in the roadway. There was no visible damage to MV1 at this time. MV2 had damage to the rear bumper. MV3 had damage to the front bumper. All cars were able to be driven from scene. All parties were offered and declined medical attention. No further incident to report at this time.

MV 2 NEVER PULLED OVER TO EXCHANGE INFORMATION AND CONTINUED GOING SOUTH ON CENTRE STREET. MV 1 SUSTAINED MODERATE REAR END DAMAGE AND NO INJURIES REPORTED. THERE WAS A PARTIAL PLATE GIVEN BY THE PEDESTRIAN CROSSING THE STREET OF 915.

Operator of MV1 states he was traveling eastbound on Lagrange St approaching Bryon Rd when MV2 made an "unusual" left turn attempting to travel southbound on Bryon Rd. Operator of MV1 states when MV2 made the left turn, MV2 collided with MV1 causing damage to the left front corner panel and drivers side door. Operator of MV2 states his tires spun out attempting to make the turn and struck MV1 with the front left corner panel causing significant damage. It should be noted the weather during this time was heavy snow with wet and slippery road conditions, which is a determining factor on how the accident happened.

MV1 was coming down the Rte 9 West off ramp to Chestnut St. Operator was traveling too fast for the snowy road conditions and failed to stop for the stop sign at the bottom of the ramp. A snow storm had started 2 hours prior to the accident and road conditions were slippery. MV1 struck MV2 broadside who was traveling north on Chestnut St with the right of way. MV1 struck MV2 hard enough that MV2 was pushed across the lane and into MV3 that was stopped at the stop sign southbound. According to the operator of MV2, MV1 was traveling "way too fast for conditions" over 30mph. Operator MV1 was cited for 89/9 failed to stop for stop sign and 90/17 speeding too fast for road conditions. Operator was also cited on Citation T1272992 for 90/23 operating after suspended license. Boylston St and Chestnut St are public ways in the city of Newton.

On Wednesday, February 13, 2019 while assigned to Traffic unit N525, I responded to the 280 Newtonville Avenue, Newton for a report of a past hit and run accident. I met with Madeleine Aubourg (S67566148) who stated she is a Nursing Assistant at Cabot Park Village. Aubourg stated due to the snowfall the area had the previous day, Aubourg left her vehicle parked (S) in the rear parking lot overnight. Aubourg stated this morning when she returned to work she observed damage to the front bumper area of her 2018 Toyota Camry (MA: 6AN179). Aubourg stated she believes sometime overnight when the parking lot was being plowed a plow truck hit the front of her vehicle. I observed moderate damage to the front bumper area. I spoke with Adolfo Merida-Diaz (S44133458) on scene who stated he is employed by Marquense Landscaping and is responsible for the snow removal at 280 Newtonville Avenue. Merida-Diaz stated last evening and this morning he removed snow from the parking lot and did not make contact with any vehicles in the parking lot during his duties. I showed Merida-Diaz where the alleged accident occurred. Merida-Diaz stated he does not remember a car being parked in that particular spot, but if there was a car parked there he leaves enough room so he is not near any vehicles. I did not observe any surveillance cameras in this portion of the parking lot. There are no witnesses to the alleged accident. Aubourg was advised to speak with her insurance company.

On 2/12/2018 at approx 1445hrs while assigned to 497 I responded to the area of 659 Chestnut St for a report of a two car crash . Upon arrival Ma Reg 9VA441 being operated by Steven HUTCHINSON and Ma Reg 98NL90 being operated by Vi DANG were pulled to the side of the road, there was a heavy snowfall and the road had accumulated a thick layer of snow and multiple crashes were occurring. NFD was on scene , DANG was crying and hysterical despite the minor nature of the crash and zero damage to either vehicle. HUTCHINSON stated they were creeping along NB on Chestnut St when DANG stopped, HUTCHINSON attempted to stop and his 4wd engaged and he slid slightly into the back of DANGS car, no marks or damage to either car. DANG elected to go to NWH with Cataldo . DANGs veh towed so as not to obstruct roadway.

On 2/13/19 at approximately 2045 hours, I responded to a single vehicle accident. The operator was driving northbound on Woodland rd and was taking a left hand turn on to Maple st when the car slipped on the snow/ice and collided with the no parking sign on the sidewalk. No injuries reported. Todys was notified to help assist getting the vehicle off of the sign. Two pictures were taken and submitted. The city was notified of the sign being down and in need of a replacement.

Operator of MV # 1 states that she took a right turn off of Parker Street on to Kendall Road when she lost control due to snow and ice on the right side of the road. The vehicle then veered off to the right over the curb and struck a United States Post Office Relay Mailbox knocking it completely over. The mailbox was located in front of 7 Kendall Road. No injuries reported by the operator and the vehicle sustained minor front end damage to the right side. I did observe an accumulation of snow and ice on the right side of Kendall Road that came out approximately one foot from the curb. Newton Center Post Office was notified regarding the mailbox. There was no mail in the box at the time.

On Thursday 2/21/19 at approximately 0653 while assigned to marked unit N499 I was travelling westbound on Nahanton Street when I observed a motor vehicle pulled over to the right side of the road with its emergency flashers on. I pulled behind the vehicle and spoke with the operator, FINCH Robert L, who states that he was travelling Westbound on Nahanton Street and due to the weather conditions he lost control of his vehicle and slid into utility pole #14. There was very minimal damage done to the utility pole, however FINCH's car was disabled due to the collision. The weather conditions were poor as it was raining and there were several inches of snow on the ground from a storm that morning causing the roads to be slushy and slippery. FINCH was uninjured and stated that he had AAA on the way to tow his motor vehicle.

The owner of MV#1 stated her vehicle was parked in the rear parking lot (Located on Rice St) of 1066 Centre St and was informed by 2 witnesses that stated her vehicle was struck by MV#2. I spoke to the 2 witnesses and they stated they were on the second floor of 1066 Centre St in suite #212 and had a bird's eye view of the rear parking lot on Rice St. The witnesses stated that MV#1 was parked while MV#2 was parked at an angle at a very close proximity to MV#1. The witnesses stated the operator of MV#2 was cleaning the snow off her vehicle, got into her vehicle then backed out of her parking space striking MV#1. The witnesses took a digital photograph of MV#2's registration (MA REG 763YC9). They described the operator of MV#2 as an Asian female in her 40's, 5'1" in height, operating a black colored "Buick" S.U.V type vehicle. MV#1 sustained minor damages to its front passenger door and bumper. A query of MA REG 763YC9 showed that it was registered to an Asian female to a black colored "Buick Rendezvous". The owner of MV#1 stated the owner of MV#2 also has a business in the same building at 630 Commonwealth Ave (Move2Boston Group). I spoke to the registered owner of MV#2 via telephone and she stated she was parked in the rear parking lot on Rice St but denied making contact with any vehicles. I could not observe MV#2 as it was not at the scene. At 14:40 I returned to 1066 Centre St and was able to observe MV#2. MV#2 displayed minor scrapes to its front driver's side bumper which was consistent relative to where both vehicles were parked. Based on the witnesses statements I will document this accident and provide this report number to the vehicle owners.

On Thursday, February 21, 2019, while assigned to Traffic unit N526, I responded to the area of Ionia Street, Newton for a report of a motor vehicle accident involving a City of Newton owned vehicle. Ionia Street is a public way maintained by the City of Newton. The weather at the time of the accident was overcast. The road surface was icy and snow covered. The operator of MV1, Patrick Carresi (MA: S73958042), stated he was operating a City of Newton owned 2016 Ford F350 plow truck (MA MVN: M94756) turning left onto Ionia Street from Auburndale Av. Carresi stated as he took the turn, the rear passenger corner of the truck made contact with the drivers side front panel of MV2 which was parked on the side of the road facing southbound. I observed minor damage to the rear of MV1. MV2 received heavy front end damage with the entire front panel torn off. Carresi reported no injuries. Carresi works for the Utilities Department with the DPW. The operator of MV2, Donna Canavan (MA: S57637058), stated she was parked inside her 2005 Honda CRV (MA: 8ZEE40) facing southbound on Ionia Street. Canavan stated as the plow truck took the turn onto Ionia Street from Auburndale Av its rear corner of the truck struck the front panel of her parked vehicle ripping the panel off. I observed significant damage to the front panel area of MV2. Canavan reported no injuries and was able to drive her vehicle from the scene. Pictures were taken of both vehicles and submitted to the IT Bureau.

On 3/4/19 I responded to the area of Hammond St and Chestnut Hill Rd for a report of a two car MVA. Upon arrival it was determined there were no injuries and the two drivers signed patient refusals with the ambulance. I first spoke with the operator of MV#1 who stated he was driving northbound on Hammond St near the Chestnut Hill T station where it bends and MV#2 was coming southbound and skidded on the snow and came into his lane. I then spoke with the operator of MV#2 who stated he was driving southbound on Hammond St when his vehicle lost control around the turn and drifted into the opposite lane and struck MV#1. I issued the operator of MV#2 MA Citation T1268628 for Chapter 89/4A; Marked Lanes Violation. At the time of this accident there were heavy snow conditions. Tody's Towing responded and towed both vehicles from the scene.

Operator of MV#1 stated he was travelling westbound on Commonwealth Ave with his dog in the front seat when he slid on ice and off the roadway. The roadway was wet from previous snow fall but was very well plowed and sanded. MV#1 hit a curb, drove through a snowbank, hit a tree with the left front of his vehicle and then hit a pole with the right front side of his vehicle. The car then came to a rest in the median. The operator appeared confused by the accident and did not realize the extent of the damage to his car.He asked what he had hit. He continuously asked if it was driveable even though the entire front end was totaled. He was able to provide me with his license and all pertinent information but spoke in a slow manner. There was no odor of alcohol. Operator declined medical attention. When asked if he was on any medication he stated he was on heart medication. We were able to contact the operator's son who was transported to the scene and stated the operator had previously suffered a stroke. David Donahue arrived on scene towed the vehicle to Tody's. A licensed driver picked up the operator. A search of the immediate area yielded negative results for ice in the roadway. A request for Immediate Threat License Suspension/Revocation was filed.

On Saturday March 9, 2019, at approximately 0230 hours, I, Officer Brandon, responded to the Weston/Newton line on Commonwealth Ave to assist Weston PD with field sobriety. On arrival Weston PD had concluded field sobriety and determined that the party, Glenn Matot, was not under the influence. Weston PD then explained that they had been contacted by AAA stating that a single vehicle accident occurred in Weston's jurisdiction. After further review, Weston PD and I determined that the accident was in fact in Newton. I then spoke with the operator of MV1, Glenn Matot, who stated that he was on his way home from work traveling westbound on Commonwealth ave in front of the Charles River Canoe and Kayak when he became distracted by a text message. Mr. Matot further explained that while texting, his car drifted to the right causing him to drive up on the snow bank and up onto the guard rail before then falling off back into the road. MV1 sustained major damage to both the passenger side and undercarriage. Mr. Matot did not sustain any injuries and denied any medical attention. AAA arrived on scene and removed the vehicle from the road and Mr. Matot took an Uber home. Two pictures were taken of the damaged City of Newton guard rail and submitted to IT to be attached to the report.

As she was pulling up to stop for the Valet, her leg hit the gas pedal instead of the brakes. On 03/12/2019, while assigned to WN, I, Officer Conary, met with DRAGAT, who wanted to report a motor vehicle accident have occurred on 03/04/2019. MV1 was parked behind 25 Oak Street in a parking stall. MV2 with a plow attached hit MV1 right rear bumper while plowing the snow. Witness, RAYMOND, asked the plow driver if they wanted MV1 moved before the accident and they said no. RAYMOND saw MV2 hit MV1. DRAGAT did not get the information from the operator or passenger of MV2. Operator of MV2 told her to contact the owner of the MV2, who was the operator's boss. DRAGAT said that she notified her insurance and the owner PADILLAGONZALES. DRAGAT said PADILLAGONZALES failed to give more information regarding the incident and DRAGAT's insurance advised her to file a police report. DRAGAT said that her insurance and PADILLAGONZALES's insurance have been in contact with one another. I attempted to call PADILLAGONZALES with negative results. DRAGAT is going to update her insurance. No further incident to report.

On 12/1/19 I was working N491 when I responded to Washington St. at Thornton St. for a repot of an MVA. Upon arrival I observed MA Reg TCY356 stuck on top of a guardrail in front of 371 Washington Street. I spoke with the operator identified as Quinn Fay who stated that while traveling Westbound on Washington St., he lost control of the vehicle in the snow and crashed over the curb and onto a guardrail. Fay and his passenger Karlis Brava stated they were both not injured and took and uber home. Todys responded and towed the vehicle.

On Tuesday, December 3, 2019, while assigned to Traffic unit N525, I responded to 364 Woodward Street, Newton for a crash involving a City of Newton owned vehicle. The weather at the time of the accident was light snow. The road surface was wet. Woodward Street is a public way maintained by the City of Newton. I spoke with the operator of MV1, Brad Butera (S94383997). Butera stated he was operating a City of Newton owned 2011 Volvo Front Loader (MA MVN: M88537) on Woodward Street while clearing the roadways during a declared snow emergency. Butera stated he was backing up Southbound across Woodward Street in the area of 364 Woodward Street after clearing some snow. Butera stated he looked and did not see any vehicles behind him. Butera stated he was backing the Front Loader up and the rear back of his vehicle crashed into the front hood area of MV2. I did not observe any obvious damage to the bucket area. Butera reported no injuries. Butera stated the operator of MV2 stated "My fault" to him during their exchange after the crash. Butera is an employee of the City of Newton DPW. I spoke with the operator of MV2, Marco Caceres (S67223904). Caceres stated he was operating his 2006 Mercedes E350 (MA: 26P270) Westbound on Woodward Street. Caceres stated he saw a Front Loader ahead of him clearing snow in the roadway. Caceres stated he stopped his vehicle in the roadway and then began backing into the driveway of his residence (Southbound) located at 364 Woodward Street. Caceres stated as he was backing into his driveway, MV1 began backing up towards him. Caceres stated he was honking his horn numerous time. Caceres stated the rear of MV1 crashed into the front hood area of his vehicle. I observed damage to the front driver side hood area. Caceres reported no injuries. I spoke with a witness to the crash, Emily Frederick. Frederick stated she was clearing snow off of her car in the driveway of 364 Woodward Street. Frederick stated she heard a car honking and looked up into the roadway. Frederick stated she observed MV1 crashed into the front end of MV2. Photos were taken of both vehicles and submitted to the IT Bureau.

On Wednesday, 12/4/2019, at approximately 2045 hours, Ofc Helms and I responded to a past hit and run at 27 Parsons St. Upon arrival, I spoke with the owner of MV 1 (Marc Rizza) who stated that he noticed damage to the left rear of his vehicle today when he came back from vacation to pick up his car. MV 1 was parked in the driveway of 27 Parsons St, (the brother in law of the vehicle owner). He further stated that he believed a plow may have struck his car when it was plowing the driveway after or during the last snow storm. Yellow streaks were visibly seen on the left rear bumper consistent with the damage. The owner of MV 1 further stated that he believed the house across the street, 28 Parsons St is the person that usually plows the driveway. I spoke with Jeffrey Timmins who told me that he in fact did plow the driveway of 27 Parsons St after the storm sometime on Tuesday 12/3/2019. Mr. Timmins admitted to being the operator of MV 2 and stated he did not notice if he hit MV 1 while plowing the driveway but he "could have". I advised both parties with an accident report number. No tows required or reported injuries by either party.

Vehicle #1 was traveling north on Chestnut St. In the area of Chestnut St. and Oliver Rd. Vehicle #1 struck the curb on its passenger side. Vehicle #1 then crossed the double yellow lines crossing into the south bound travel lanes. Vehicle #1 then crashed into and over the curb and snow bank traveling completely off the road and into a utility pole on the sidewalk. Vehicle #1 was sustained heavy front end damage and was elevated off the roadway on top of a snowbank with a utility pole indented into its front end. The crash into the utility pole caused the pole to split in half. The operator of vehicle #1 was uninjured and refused medical attention. The operator of vehicle #1 was cited with MGL 90-24/K operating under the influence, MGL 90-24/E negligent operation of a motor vehicle, 90-24/C leaving the scene of property damage and MGL 89-4A marked lanes violation (citation #T2079135 & #T2079136). For validation purposes it should be noted that entering citation #'s in the person tab, only one citation # could be entered. I observed heavy front end damage to the front end/passenger side front of the vehicle and it was towed by Tody's. There was no damage to the utility pole. There is a decline in elevation to the roadway in this area and the road conditions were slippery due to the current snowstorm that was producing snow and freezing rain at the time.

On the above date and time I responded to 717 Centre St for a report of a crash involving two vehicles. The weather was snowy / rainy/ sleeting, and the roadway was a mixture of ice/ snow. Upon arrival on scene I spoke with the operator of V1 Ms. Howe, she stated she was traveling south on Centre St when V2 stopped abruptly. Ms. Howes stated she applied the brakes and her vehicle would nt stop due to the icy roadway and she had contact with V2. I then spoke with the operator of V2, Ms. Nunnally stated that a vehicle had abruptly pulled out of the driveway of 717 Centre St. She stated she applied her brakes immediately to avoid contact and pulled to the right. She stated when she stopped she felt rear impact from V2. Both parties had no injuries at this time. V2 was towed by Todys towing to 1354 Washington Street Newton. Ms. Howe removed valuables from the vehicle. At this time it appears the icy roadway was a contributing factor of the crash.

On 12/19/2019 while assigned to WNSQ I took a report at the front desk of NPD HQ for a past single motor vehicle cash. On 12/19/2019 Vehicle 1 was on Exeter St attempting to turn around in the driveway of 1536 Commonwealth Ave. While entering the driveway Vehicle 1 slid on a patch of ice and exited the roadway. The vehicle came to final rest in the front yard of 1526 Commonwealth Ave. No injuries were reported. No vehicles required a tow. Vehicle 1 had no visible damage. It should be noted that at the time of the crash the weather was clear with freezing temperatures. The area had just experienced snow and freezing rain precipitation on 12/17/2019. The operator of Vehicle 1 stated that after sliding into the front yard she was able to drive Vehicle 1 back on to Exeter St. The operator of Vehicle 1 stated that while she was exiting the front yard her tires were spinning and may have caused minor damage to the lawn. The operator of Vehicle 1 stated that she attempted to notify the homeowner of 1526 Commonwealth Ave but nobody appeared to be home. Officer Claflin N493 responded to 1526 Commonwealth Ave and tried to raise the homeowner with negative results. Officer Claflin observe tire tracks in the front yard but due to the presence of snow and ice it is unknown how much damage had been done. No phone number is listed for the homeowner of 1526 Commonwealth Ave as a result I will mail notification of this crash to the homeowner.

On Thursday, January 23, 2020 while assigned to Traffic unit N525, I responded to the intersection of Brookline Street and Hartman Road, Newton for a report of a motor vehicle/pedestrian crash involving a City of Newton Crossing Guard. The weather at the time of the crash was overcast. The road surface where the crash took place was dry. Brookline Street and Hartman Road are both public ways maintained by the City of Newton. On arrival, the injured pedestrian was in the process of being transported to Newton Wellesley Hospital with minor injuries. I spoke with the operator of the vehicle involved in the crash, Menachem Uminer (S43996134). Uminer stated he was operating his vehicle on Brookline Street (S) towards Hartman Road. Uminer stated he was stopped in traffic on Brookline Street prior to the crash. Uminer stated the vehicle in front of him proceeded to travel straight from it's stopped position and he then began to follow the traffic on Brookline Street. Uminer stated he did not see anyone in the vicinity of his vehicle as he began to travel straight from his stopped position. Uminer stated as he was traveling by Hartman Road, the front passenger side area of his vehicle made contact with a pedestrian on the right side of Brookline Street at Hartman Road. Uminer stated he did not see this party until after they were struck and he then proceeded to pull his vehicle over. Uminer stated he was operating his vehicle at a very low speed at the time the collision took place. After speaking with Uminer, I observed the white 2017 GMC Yukon (MA: 9NX525) involved in the crash parked on Brookline Street (S) just past Hartman Road. Uminer stated the vehicle had been moved after the collision took place. I observed no obvious damage to the body of the vehicle on the front passenger side area where the collision took place. The front passenger door mirror was folded inward towards the door. Uminer stated he believes the front passenger door mirror was the point of contact between his vehicle and the pedestrian. I spoke with a witness to the crash, Ashley Garcia (S94193596). Garcia stated she was attempting to cross Brookline Street at Spaulding Lane. Garcia stated she entered the marked crosswalk and was a few steps into Brookline Street when a black colored vehicle traveling on Brookline Street (N) slammed on its brakes near the crosswalk. Garcia stated she looked to her left at the vehicle and then heard a thud to the right from the area where the crossing guard was standing near Hartman Road. Garcia stated she looked over and saw a crossing guard on the roadway on Brookline Street (S) at Hartman Road. The injured pedestrian involved in the crash is Cassandra Alexander (S23636307). Alexander is a crossing guard for the City of Newton and is assigned to the intersection of Brookline Street and Hartman Road for the Memorial Spaulding school crossings. This crash occurred while Alexander was performing her official duties. Alexander was wearing her department issued yellow reflective jacket at the time of the crash. I spoke with Alexander at Newton Wellesley Hospital where she was being treated for her injuries. Alexander stated she was performing her duties as a crossing guard at the intersection of Brookline Street and Hartman Road. Alexander stated she had just finished crossing two pedestrians in the marked crosswalk on Hartman Road when observed a pedestrian enter the crosswalk on Brookline Street at Spaulding Lane. Alexander stated she began to enter the intersection of Brookline Street at Hartman Road with her hands up and her department issued yellow reflective jacket on in an effort to stop traffic in the area to allow the pedestrian in the marked crosswalk safely cross. Alexander stated she was struck on her left side by a passing vehicle. Alexander stated she did not see the vehicle and fell to the roadway. Alexander stated as she was falling to the roadway she extended her right arm in an effort to brace her fall. Alexander stated she had injuries to her arms and legs and was sore from the collision. I spoke with a second witness to the crash via telephone, Anand Vaidya (S43996134). Vaidya stated he was stopped in traffic behind MV1 on Brookline Street (Southbound). Vaidya stated he observed the vehicle in front of MV1 proceed straight on Brookline Street after being stopped in traffic. Vaidya stated he then observed MV1 proceed straight on Brookline Street at Hartman Road at a low rate of speed. Vaidya estimated the speed of MV1 at this time to be 5 miles per hour. Vaidya stated a crossing guard was on the right side of the roadway on Brookline Street at Hartman Road wearing a yellow and black reflective jacket and snow pants. Vaidya stated the crossing guard had her back towards him and MV1, and extended her arm out to stop traffic. Vaidya stated at this time MV1 rolled by the crossing guard at a low rate of speed and the front right fender area of the vehicle hit the left rear leg/lower back area of the crossing guard. Vaidya stated the crossing guard bounced off of the car and fell to the roadway. Vaidya stated he believes MV1 was unaware he hit the crossing guard and observed the vehicle travel another 10 feet until it came to stop on Brookline Street. Vaidya stated the crossing guard was not in a marked crosswalk at the time of the crash. After speaking with all parties involved, Uminer will be cited with Massachusetts Uniform Citation T0115905 for Chapter 90, Section 14 (Failure to slow for a pedestrian in the roadway). Photos were taken of the vehicle and scene and submitted to the IT Bureau.

On Friday, February 7, 2020 while assigned to Traffic unit N525, I responded to the rear parking lot of 1 Boylston Street, Newton for a report of a hit and run motor vehicle crash. I spoke with the operator of MV1, Hong Zhang (S39408212). Zhang stated while operating her 2019 Volkswagon Tiguan (MA: 6YJF90), she was attempting to back out of a parking spot in the parking lot behind 25 Boylston Street, Newton. Zhang stated while backing out of the spot she crashed into an unknown vehicle traveling Westbound through the parking lot. Zhang stated the only information she had for the other vehicle was that it was a large red and black truck. I observed damage to the rear trunk window of MV1. Zhang reported no injuries. Zhang stated she was unsure if she hit the vehicle or if something fell off of it as it was passing by. I went to several local businesses in the area in an attempt to view surveillance footage of the area where the accident occurred with a negative result. While canvassing the area for the other vehicle involved, I observed a red dump truck (MA CON: T71619) removing snow on the property next to Hammond Pond Parkway. The operator stated he has been working down that end of the parking lot for most of the morning and stated he was not involved in a crash. I observed no fresh damage to the vehicle consistent with a crash.

Over the weekend there was snow that needed to be removed and due to my training experience and the bright yellow paint transfer and how high the damage was to both vehicles, it is believed to be a plow. Vehicle one had significant damage to the front passenger side wheel well and passenger door. Vehicle two had a long deep scratch from the passenger side front door to the rear door. Both vehicles showed bright yellow paint transfer onto the cars.

On 12/05/2020 at approximately 22:00 hours I was called in to respond to a single vehicle crash with serious injuries in the area of 291 Nahanton St. Nahanton St is a pubic way in the City of Newton. Upon my arrival at the crash scene with Sgt Devine I was briefed by Officers Daly and Anderson, ( see Ofc Daly's report for further information CAD # 20054728 in regard to changes take out against the operator of the truck ) I made the following observations of the crash scene, Nahanton St is a public way in the City of Newton that is a two way roadway divided by an unprotected merdium ( double yellow line ) that runs east to west. The roadway surface was wet due snow and ice. The roadway was plowed and the snow was pushed to the shoulder, althought wet the roadway was icy. There was approximately 2 inches of snow on the shoulders of the roadway. The road was light with overhead head street lights. The posted speed limit is 25 MPH. I observed a 2013 Chevrolet Silverado pick up truck that had major front end damage. The Chevrolet was at an angle with it's front end facing west bound and resting on the grass shoulder and into a wooded area. The Chevrolet rear was sticking out into the shoulder of the east bound lane of Nahanton St. The passenger side front and rear doors of the Chevrolet had been removed by the fire department to extricate the driver and passenger. The driver and passenger side air bags were deployed . The front passenger side windshield of the Chevrolet was completely missing and the driver's side had completely shattered. The front tires, hood, and engine had been compacted on it's self. The front and rear driver's side doors had been mechanically open by the fire department. The speedometer needle had stopped at 54 MPH. The Chevrolet left the roadway and left tire marks in the snow in the eastbound bike lane of Nahanton St. At a 106.4 feet from the beginning of the tire marks the first point of impact was with two tree stumps that were 3.5 feet from the roadway, four feet tall and 3.5 feet in diameter. The tire marks continue in the east bound bike lane and exit the roadway and onto a dirt shoulder at 136.1 feet from the beginning of the tire marks. The truck continued to the right of a utility pole and then it's passenger side made contact with a tree at 170.8 feet from the begin of the tire marks. The tree was 12 feet from the bike lane approximately 5 feet in diameter. The impact with the tree caused the truck to turn so that the rear was facing north east on the dirt shoulder and bike lane. The final point of rest of the rear passenger side tire was 200 feet from the beginning of the tire marks. The truck's rear diver side tire final point of rest was 209 feet from the beginning of the tire marks. The front driver's side tire was compacted into the trucks engine and the passenger side front tire was located behind the tree approximately 20 feet from the roadway. Sgt Devine took photos of the crash area and of the truck and the disk was download by the NPD's IT Bureau. The truck was towed from the crash scene by Todys's Towing to their lot for storage, I fill out a towed motor vehicle form. There were no reported witness who observed the crash. The crash was reported by an unknown passerby. On 12/07/2020 Ofc Gaudet faxed a request for preservation of medical records, blood and urine samples of Schneider to Beth Israel Deconess Medical Center.

MV1 was parked in front of Silver Lake Laundromat in the parking lot at 230 Adams St. facing Eastbound. Owner of MV1 stated she was inside folding laundry between 2:30AM and 3AM. Stated that she discovered that her driver side head light was damage and had fallen off her vehicle around 3:30AM when clearing snow off her vehicle. Owner of MV1 stated that she believes that the driver of an unknown white truck that was plowing the parking lot had backed into her vehicle stricking the driver side headlight of her vehicle. I asked the owner if she saw the incident occur which she stated "no." I also if she had the plate or had any other description of the truck, which she replied "no." Owner of MV1 stated that she spoke to the operator of the white truck, and described him as a tall, older white male who was wearing a black hat and had dark mustache. This person was not on scene when I arrived and the owner of MV1 stated the operator of the unknown white truck stated that he had not hit her vehicle. Due to snow from an on going snow storm, I did not witness any vehicle tracks by her vehicle. There were also no security cameras in the parking that would of recorded the incident. On 12/17/20 I responded to #964 Boylston St- The B.P. Gas station for an accident that happened this morning at 02:35. The owner of the gas station Elias Ghazale showed me video that showed a white pick-up truck with a plow that pushed snow across Ramsdell St onto a customers car. When the truck pushed the first pass of snow, the security camera shows the entire vehicle move violently into the curb. It is unknown at this time if there is any damage to the customers vehicle. It is also unknown at this time who was operating the plow or the name of the company. Because of all the snow plowed up on the customers vehicle, the license plate and information on the vehicle is also unknown at this time. I traveled to Negoshians Towing and left a message asking the owner to call me at the station to follow up on this matter. On 12/18/20 I spoke with Elias Ghazale who gave me a Mass. Registration of 1WPG55 a 2009 Honda Odyssey grey van that is owned by a Sarkis E. Ghazale as the owner of the parked vehicle that was involved in this accident. I traveled to Ferguson Plumbing and spoke with the manager Eric Mackenzie who stated the landlord, Negoshians Towing deals with the snow removal for his property. I then traveled to Negoshians and spoke with the Owner Ted Negoshian. He stated his company is responsible for plowing the Ferguson lot. I showed him a security clip of the white truck plowing in the parked vehicle and Negoshian stated that he doesn't have any white trucks that plow for him only black ones. I then traveled to the City of Newton Highway Department and spoke with Superintendent Michael Arpino. I showed him the security video and he stated that the white truck in question was not a city owned or sub-contracted vehicle because the trucks would have a green flag on their mirrors or writing on the door. I also spoke with Newton Control Clerk Mark Parsons who viewed the City of newton GPS from the night of the storm and confirmed that no city vehicles or sub-contracted vehicles were on Ramsdell St between 2am and 3am. Unless new information on the identity of the white truck is learned, the vehicle in question (veh#2) will remain unknown. On 12/28/20 I spoke with Elias Ghazale's wife, Maria Vidalis wife who stated she was able to uncover the snowed in vehicle and found the following damage to Massachusetts Reg # 1WPG55: drivers door, front bumper, rear quarter panel scratches, drivers side fender and reports of leaking transmission fluid.. I then traveled to #964 Boylston St and viewed the vehicle myself and all confirmed the above damage except the transmission fluid.

On Monday, December 21, 2020, while assigned to Traffic unit N525, I responded to 8 Cragmore Road, Newton for a report of a motor vehicle crash involving a City of Newton Fire apparatus. Cragmore Road is a public way maintained by the City of Newton. The weather at the time of the crash was cloudy. The road surface was wet from melting snow in the area. The operator of MV1, Jon Papia, stated he was operating City of Newton Fire Engine 7 (MA MVN: MF470 ) Southbound on Cragmore Road. As MV1 travelled past 8 Cragmore Road, it's passenger side made contact with the front driver side area of MV2 (MA: 1JTC19). The Honda Accord was parked and unoccupied at the time of the crash. The registered owner was made aware of the crash and advised. I observed no damage to the passenger side of MV1. I observed damage to the front driver side of MV2. Photos were taken and submitted to the IT Bureau. No injuries were reported by any of the involved parties.

On 12/24/20, while assigned to N493, I, Officer Conary, responded to the area of Westwood Street for a report of a hit and run that just occurred. I was already in the area and did not observe any vehicle driving fast. Upon arrival, I met with Owner of MV1 who stated he was inside at 21 Westwood Street when he heard crash. He looked outside and saw a white sedan, possibly a BMW, quickly traveling Northbound on Westwood Street. MV1 was parked right outside.There was damage to the rear of MV1. I saw surveillance video from 21 Westwood Street, which showed a white sedan reversing down the street quickly and hitting MV1 then continue to drive away. MV1 did not require a tow. There was no reports of injuries. Following is an update by Officer M. Boudreau: At 12:07 On 12/25/20 I was dispatched to 95 Pearl Street because the vehicle involved in the crash had been found. MA Reg#1VYW99 a white BMW M3 was parked on Pearl Street and had damage to the rear of the car. Based on the video I was shown the damage is consistent with the accident and it was the same type of car. The family of owner #1 asked around the neighborhood and learned that Qiushi Wang had been driving the vehicle. I spoke to the owner of vehicle #2 who eventually admitted that his son had been driving vehicle on Learners Permit. The father Ningshan Wang stated that he was also in the vehicle as a Licensed Operator. Qiushi Wang stated he thought he hit a snow bank, then stated maybe he wasn't driving at the time, he eventually admitted to hitting the parked car. Qiushi also was shown video and admitted it was his car in the video. Asked video be sent to my email or brought into NPD Both father and son will be issued citations for leaving the scene of property damage. All three operators attempted to bypass a detour that was put in place at the intersection of Centre Street and Cabot Street to prevent vehicles from traveling Northbound on Centre Street due to heavy amounts of snow. When MV1 took a right and then a left turn onto Sergeant street to bypass N 503 with all emergency lights activated in an attempt to merge onto Centre street, MV2 and MV3 followed. On Wednesday February 4, 2021 at approx. 0901 hours I responded to Washington Street at Perkins Street for a report of a two motor vehicle crash with no injuries. Upon arrival I observed the crash was during daylight hours, weather was cloudy and the roadway was wet due to recent snow fall that was cleared. I spoke with operator one Mr. Smith he stated he was in the left travel lane heading east on Washington Street when vehicle one swerved left into his lane to go towards Mass pike entrance ramp and had contact with his vehicle. Operator of vehicle two Ms.Bovell stated she was traveling east on Washington Street and observed vehicle one traveling in the middle of both east bound lanes of Washington Street. She stated he then merged left traveling in the left lane of Washington Street and she traveled in the right lane. She stated while traveling vehicle one merged into her lane and side swiped her in the left rear. Upon obtaining all documents, vehicle one operated by Mr. Smith was missing his registration plates, registration and any documents of status of the vehicle. Upon further investigation Mr. Smith had a suspended license of January 26, 2021, in addition the vehicle vin number(1FT7X2B68BEC58523 - 2011 FORD TRUCK ) came back inactive on December 16, 2020, which was allegedly sold to Mr. Smith. in the middle of December 2020. Mr. Smith had no paperwork on the vehicle, therefore vehicle one was towed by Todys Towing 1354 Washington Street Newton and will be impounded until proper documents on owner ship are verified. Mr. Smith was issued in hand citation number # T2017265 for the following 90/23 oper w/ susp lic 90/9 oper w/unreg mv 90/20 insp sticker All parties advised of the process and no injuries reported at this time. I filled out the proper paper work for towed vehicle.

She reported going S/B on Bridge St when vehicle #2 came out into the intersection from Chandler St crashing into her vehicle. Operator #2 identified as Carly Lazarus was operating a grey 2015 Honda civic. She reported being on Chandler St E/B and as she approached the intersection of Bridge St she attempted to stop but due to the snowy surface slid out into the intersection striking vehicle #1. Neither vehicle needed to be towed.

On Tuesday February 9th, 2021 at approximately 1334 hours I responded to the intersection of Ascentra Terrace and Highland Street for a two car motor vehicle accident. At the time of the accident the weather condition was heavy snow in the area and the roads were covered in snow. Both Ascentra Terrace and Highland Street are owned and maintained by the City of Newton. Upon arrival I met with the operator of MV1 identified as Elizabeth Carron (S53478024). She stated she was heading northbound on Highland Street when she lost control of her vehicle on the bend of the road and made contact with MV2. MV1 sustained heavy rear end damage and airbag deployment. The operator of MV2 identified as David Balaban (S09328852) stated he was traveling southbound on Highland Street and could not avoid MV1 when it lost control and collided with his vehicle. MV2 sustained heavy drivers side panel damage. The Fire department and Medic 2 arrived on scene and cleared with a patient refusal. Todys responded and towed MV1 as it was inoperable.

On 2/12/2021, at 2315 hrs, Newton Police were dispatched to the area of Watertown St. and Brookside Ave. for a single motor vehicle collision. Upon arrival, MV#1 (Red, Jeep Grand Cherokee) was observed parked, on the side of the road facing eastbound in front of 687 Watertown St. MV#1 was observed to have front end damage and two flat tires (driver side). The operator, who was standing outside the vehicle, stated he was traveling eastbound on Watertown St. when he became distracted, lost control of the vehicle and collided into the snow bank.The driver did not report any injuries. The vehicle was towed from the scene by Perfecton Towing of Waltham, Ma. Damage to MV#1 does not appear consistent with the operators recollection of events, but a check of the immediate area by Newton Officers did not yield any damaged property. Damage to MV#1 was photographed with the (N492) camera and will be attached to this report.

On 02/25/2021, while assigned to N494, I, Officer Conary responded to 41 Algonquin Road for a motor vehicle that struck a parked motor vehicle. The weather was cold outside and the ground in the area had iced over. I spoke with Operator of MV1 who stated he was driving Northbound on Algonquin Road when he lost control of his vehicle due to the ice on the road, and hit MV2 and then hit a snow bank. I asked he wanted medical attention and he declined. There was damage to MV2 left rear bumper and tire. MV1 had damage to the front bumper. MV2 remained parked on scene, and MV1 was able to be driven. Owner of MV2 and Operator of MV1 exchanged necessary paperwork. City was notified of the ice and also debris in the road.
