Operator of vehicle one stated that while driving east bound on Crafts St at Albemarle Rd vehicle two darted out into the path of his vehicle from the south bound lane of Albemarle Rd. Operator of vehicle one stated that he attempted to avoid a crash with vehicle two by applying his vehicle's brakes and swerving to his left. Operator of vehicle one stated that his vehicle's front bumbler made contact with vehicle two's front driver's side fender and bumper. Operator of vehicle one stated that he was not injured his vehicle was not towed. Operator of vehicle two stated that he had stopped at the stop sign in the north bound lane of Albemarle Rd at Crafts St. Operator of vehicle two stated that he saw vehicle one driving east bound on Crafts St and he pulled out into the east bound lane of Crafts St in font of vehicle one believing that he had enough time to cross over Crafts St . Operator of vehicle two stated that vehicle one's front bumper struck his vehicle's front driver's side. Operator of vehicle two stated that he was not injured and his vehicle was not towed. Passenger in vehicle two Zbigniew Saletnik stated that he was not injured and had little recollection of the crash. All Streets are public ways in the city of Newton. I issued operator of vehicle two MA Citation T0340520 and cited him for MLG 89/8 , failure to yield. I took photos of the vehicles and crash area, the disk was downloaded by the NPD's IT bureau.

Narrative:
On Wednesday, January 9, 2019, while assigned to Traffic unit N525, I responded to 457 Walnut Street, Newton for a report of a past motor vehicle/pedestrian accident involving a student at Newton North High School. I met with the pedestrian involved in the accident, Ellie McBride, in the Riley House admin office. McBride is a Freshman at Newton North High School. McBride's mother, Linda Hanusaik, was also present while I spoke with her. McBride stated at approximately 0740 hours, she was attempting to cross Lowell Avenue in a marked crosswalk at Highland Street (E). McBride stated she observed a newer model gray Acura SUV traveling towards her Southbound on Lowell Avenue while she was standing in the crosswalk. McBride stated she thought the vehicle was going to slow down for her. McBride stated as the vehicle got closer it appeared to be slowing down but was unable to avoid making contact with her. The front end of MV1 made contact with McBride's left side. The impact caused McBride to fall down onto the roadway. McBride stated the vehicle pulled over and the operator exited the vehicle to check on her. McBride stated the operator was a white female with long brown hair, possibly high school aged. McBride stated after she told the operator that she wasn't hurt, the operator returned to her vehicle and left the area, possibly towards Commonwealth Avenue. McBride could not provide any additional information about the operator of the vehicle involved. McBride stated she then walked to Newton North High School. McBride stated she went to her morning class but left and texted her mother to tell her she was involved in an accident. McBride stated she was shaken up by the accident, but reported no injuries. Newton Medics evaluated McBride and obtained a patient refusal. McBride was advised that due to the reported age of operator, they may be a student at the high school. McBride was advised that if she sees the operator to obtain her information/contact NPD. I checked the area of Lowell Avenue and Highland Street for the vehicle involved/witnesses with a negative result. A portion of Newton North's athletic fields are next to this intersection. Newton North stated there are no surveillance cameras in this area. Lowell Avenue and Highland Street are both public ways maintained by the City of Newton.

On Thursday January 10th, 2019 at 0645 hours, I responded to a two car motor vehicle accident. There were no injuries. The first vehicle (Mass Reg # 674AX7, Chev Cobalt, Owned/Operated by Meagan Descovich) was traveling east on Webster St approaching a stop sign. The second vehicle (MassReg # 6CG475, Honda Fit, Owned/Operated by Kathryn Keniston) was traveling north on Cherry St. The first vehicle did not stop at the stop sign and collided into the second vehicle. The operator of the first vehicle admitted it was her fault and she did not see the stop sign. Both vehicles suffered major damage. Todys towed the Chevy Cobalt. Medics did not respond. Both vehicles have insurance.

The operator of MV#1 stated that she was waiting to exit NWH at the east exit/entrance. When her light turned green she proceeded to enter Washington St and her MV was struck by MV#2, who was traveling south on Washington St. The operator of MV#2 stated that she was traveling south on Washington St approaching NWH and had a green light to proceed thru the intersection at the east exit/entrance. An independent witness stated that she was in a MV directly behind MV#1 at the east exit when she observed their traffic light turn green and both her and MV#1 proceeded into the intersection. The witness then observed MV#2 traveling south on Washington St enter the intersection crashing into MV#1 while their light was still green. Operator #2 cited for a red light violation.

Based on observations and statements made, the following occurred I observed. Upon my arrival, I observed M/V#2 stopped in the right land of Ellis St. aprox. 20 feet south of the Rte 9 overpass, and M/V#1 was stopped by the curb, in the left lane, aprox. 40 feet south of the intersection. The operator of M/V#1 stated that he came off Rte. 9, stopped at the stop sign, then turned right (south) on to Ellis St. where M/V#2 collided with it. The operator of M/V#2 stated that she was traveling south on Quinnobiquin Rd, and proceeded through the intersection, when M/V#1 turned right, in front of her, and they collided. I spoke to a male party, who was seated in a car, in a parking area on Ellis St. who stated that he heard the accident, but did not see it.

Operator of MV1 (Mass reg. 8928AN) states he was traveling south bound on Waverly Ave when MV2 went through the stop sign and entered the intersection causing him to collide with MV2. Operator of MV2 (Mass reg. 7SH894) states she does not have much recollection from the accident. She states she had glare in her eyes when she was traveling east bound on Franklin Street and stopped at the intersection with Waverly Ave and proceeded through the intersection and collided with MV1. All parties were evaluated by Cataldo paramedics, operator of MV2 was transported to Saint Elizabeth's Hospital with minor injuries. Tody's towed both motor vehicles.

On Wednesday, February 27, 2019 while assigned to Traffic unit N525, I responded to the area of 843 Beacon Street, Newton for a report of a 2 car motor vehicle accident. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1, Omer Chaudhry (NY: 122909834). Chaudhry stated he as operating his 2015 blue Mazda 3 (NY Reg: GVJ1096) Eastbound on Beacon Street towards Centre Street. Chaudhry stated he observed MV2 exiting a parking spot on his right. As MV2 attempted to merge into traffic it's front driver side crashed into the passenger side of his vehicle. I observed damage along the passenger side of Chaudhry's vehicle from the rear passenger tire to the front passenger bumper area. Chaudhry reported no injuries at this time. I spoke with the operator of MV2, Elizabeth Libert (S75526057). Libert stated she was attempting to pulled out of a marked parking spot (E) on Beacon Street when the front driver side of her 2017 gray Mercedes E400 (MA: 1YT269) crashed into the passenger side of MV1. I observed damage to the front driver side tire area of MV2. Libert reported no injuries at this time. Libert's vehicle was removed from the roadway by Tody's towing. Libert was cited with Massachusetts Uniform Citation T1269154 for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care).

On 3/5/19, I responded to Crafts St at Albemarle Rd for a motor vehicle accident. The operator of MV1 (NJ Reg: K89JJU) stated that she was travelling eastbound on Crafts St when MV2 suddenly attempted to drive across the street in front of her. MV1 stated that she attempted to brake but could not do so in time. The operator of MV2 (NH Reg: 4253062) stated that she was stopped at the stop sign on Albemarle Rd when she looking right and saw MV1 driving down Crafts St. MV1 stated that she thought she had enough time to make it across the street, but did not, resulting in the accident. Both parties declined medical attention. MV1 sustained moderate front end damage. MV2 sustained moderate passengers side damage. Neither vehicle required a tow. MV2 was issues MA Uniform Citation T1272249, c89/9.

On 03/07/19 at 08:29 hours, I responded to a two car motor vehicle accident at Needham & Oak Streets. The operator of M/V # 1 states she blacked out upon impact of the accident. She was immediately transported to Newton Wellesley Hospital by Cataldo Ambulance for a possible head injury. After at the hospital, Operator of M/V # 1 states she was traveling straight ahead (WB) on Needham Street at Oak Street with the green light signal in front of her. As she approached the intersection, M/V # 2 suddenly attempted a left turn from the opposite side of the road and she was not able to stop in time. M/V # 1 sustained passenger front end damage and the vehicle was towed by Tody's as the operator was transported to the hospital. Operator of M/V # 2 states she was in the left turn lane on Needham Street facing eastbound attempting to take a left turn on to Oak Street. She states that she thought it was clear to proceed and M/V # 1 came out of no where and struck her on her rear passenger side. At first, the operator told me the signal light was green and then after stated that the light was changing to red when she was proceeding through. The vehicle sustained rear passenger side rear wheel and axel damage and it was towed by Tody's. Based on both operator statements, it appears that the operator of M/V # 2 did not yield the right of way and attempted to turn left when it was not clear to proceed. The operator was issued and mailed MA Uniform Citation # T1441703 for Ch. 89/8.

On 3/10/2019(cid:31) at approx 1118hrs while assigned to 497 I responded to the intersection of Chestnut St and Amherst Rd for a report of a two car crash. upon arrival both vehicles were up on the curb at the northeast corner of Chestnut St and Amherst Rd. I spoke with the operator of Ma Reg 4BC783, UNSITAHARUTAI,Saifoun who stated she was travelling EB on Chestnut St when the operator of Ma Reg 5ES474, CANJURA, Iris cut across in front of her causing collision. Saifoun stated her father UNSITAHARUTAI, Somchai was in the front passenger seat and now complaining of neck pain. Saifouns mother , UNSITAHARTAI, Saipan was in the rear passenger seat and had no injuries. Please note Saipans only DOB is the year 1952, I added January 1St for report validation purposes.I spoke with Iris who stated she was crossing Chestnut St from Tamworth St to Amherst Rd when she saw Saifoun "going really fast". Iris and Samchai were transported to NWH by Cataldo Ambulance. Todys towed both MVs.

Narrative:
On Friday March 15th 2019 at approximately 0730 hours I responded to the intersection of Beacon Street and Herrick Road for a report of a hit and run involving a bicyclist. The weather at the time of the accident was cloudy and the road conditions were dry. Both streets are public ways in the City of Newton. Upon arrival I observed the victim identified as Glenn Butcher (MA OLN S36552335), inside the ambulance getting medical attention. Butcher was getting a bandage applied to his left leg and complained of leg, back and neck pain. Butcher appeared visibly shaken and in pain. Butcher stated he had the green traffic light and was travelling eastbound on Beacon Street crossing Centre Street and then continuing straight crossing Herrick Road. Butcher stated MV1 was travelling westbound on Beacon Street and took a left turn onto Herrick Road and collided into his bicycle. Butcher states upon impact he flipped over his bicycle and landed on the hood of MV1, before rolling off and landing on the ground. Butcher states he was wearing his helmet which was present. Butcher states his bicycle ended up partially under MV1. Butcher states the operator of MV1 pulled his vehicle to the side of the road and got out of his vehicle and spoke to several witnesses briefly. Butcher states the operator then left the scene without properly exchanging information and did not wait for the police to arrive. Butcher described the vehicle as a dark gray Honda, and described the operator as an older white male, 5'10 approximately 80 years old with white hair. Butcher was also able to provide an email address for another bicyclist who witnessed the accident. The witness left his information with Butcher prior to my arrival. Fire fighters on scene were able to get the registration number of the vehicle from the witness (MA REG 4CX148) and gave it to Officers on scene. Butcher was transported by Medic 1 to NWH with minor injuries. His bicycle had visible damage to the frame area and skid marks were present on both tires. 19 photographs of the scene and the bicycle were taken and sent to the IT dept. email. Officer Gabriel transported the bicycle to the station where it was logged into Property & Evidence for safekeeping. Dispatch was able to query the registration which came back to a gray, 2010 Honda Accord registered to a Matthew Sugarman (MA OLN S38601397), out of W. Roxbury. Dispatch contacted the Boston Police Dept. and had them send an Officer to the address to try to make contact with Sugarman. Sugarman was advised to contact the Newton Police Dept. and I was able to speak with him on the telephone. Sugarman states he was driving the vehicle today and was involved in an accident with a bicycle. Sugarman states he was travelling westbound on Beacon Street and was taking a left onto Centre Street not Herrick Road. While speaking with Sugarman he seemed confused as to his location during the accident and was adamant that he was turning left onto Centre Street and not Herrick Road. I advised Sugarman that I was on scene with the first responding Officers and the accident clearly occurred at the intersection of Beacon Street and Herrick Road based upon the final resting place of the bicycle. Sugarman went on to state that his vehicle was already turning when the bicycle struck his vehicle and he believed it was the bicyclists fault. I advised Sugarman it appears the bicyclist had the right of way since it was traveling straight and that because his vehicle was taking a left turn, he failed to yield for on coming traffic at an intersection. When asked why he did not stay at the scene of the accident Sugarman stated he got out of his vehicle and spoke to the bicyclist and he "seemed okay". Sugarman states he then left the scene while the victim was still sitting on the ground. I asked Sugarman if he had ever been involved in accident before and he replied "no." I advised Sugarman that I am currently looking at his drivers history and see nine accidents that he was involved in. Sugarman stated "he has never been in accident like this before." I then asked Sugarman what the law states when you are involved in an accident and he was not able to provide me with an answer. I advised him that the law states you must stop and exchange information before leaving the scene of an accident. Again he stated the bicyclist seemed okay and "he had somewhere to be so he left." Sugarman has an extensive drivers history including nine surchargeable accidents, reckless operation, two different charges of leaving the scene property damage and two immediate threat affidavits filed against him. I advised Sugarman that I will be mailing him a citation for Ch89 s8 Fail to yield to oncoming traffic, and Ch90 s24 Leaving the scene of personal injury. I also filed an immediate threat form with the registry. I emailed the witness using the information he provided to the Newton Fire Dept that was on scene and received a phone call back from the witness identified as Bruce Adams. See witness report for his statements. I contacted the Bank of America located at 808 Beacon Street in Newton, as they are the closest business to that intersection with video surveillance. I spoke with George Walsh (781-834-8275) who stated they do not have any working cameras on that side of the building to the bank. For this report the bicyclist is entered as a non-motorist for validation purposes. Witness Bruce Adams (MA OLN S96398108) stated that he was riding his bicycle travelling behind the victim about 100 feet away. Adams states he was watching the victim and had a clear view of him as he travelled behind him, through the green traffic light heading eastbound on Beacon Street. Adams stated he witnessed MV1 travelling westbound on Beacon Street take a sharp left turn onto Herrick Road which turns into Union Street. Adams stated that the vehicle turned left colliding into the bicycle and there was no possible way the bicyclist had time to stop to avoid the collision. Adams stated the victim flew over his handle bars and fell on top of MV1's hood and then landed on the ground. Adams stated the victim's bicycle was stuck under MV1's front bumper. Adams stated the operator of MV1 backed his vehicle up with the bicycle still attached to his vehicle bumper and pulled MV1 over to the side of the road. The operator then got out of his vehicle and walked up to the victim who was laying on the ground visibly in pain. Adams stated the operator asked the victim if he was okay to which the victim did not immediatley reply as he was visibly shaken up. Adams stated the operator then walked back to his vehicle and attempted to leave the scene and that is when Adams stood infront of MV1 to stop him from leaving the scene. Adams stated he then removed the bicycle from underneath the bumper of MV1 and placed it on the side of the road. Adams stated the operator got out of his vehicle again and walked up to the victim and asked him "what do you want to do?". Adams stated the victim did not respond and that is when the operator got back into his vehicle and drove away, leaving the scene before any emergency response arrived. Adams stated he was able to memorize MV1's registration and stayed with the victim until police, fire and medics arrived. He stated he gave the fire dept the registration and his contact information prior to leaving the scene.

On Sunday 3/17/19 at approximately 1606 while assigned to marked unit n499 I was dispatched to the area of 89 Wiswall Rd. for a report of a two car motor vehicle crash with no injuries. Upon arrival I spoke with WONG Cheung, the operator of motor vehicle #1. WONG states that he was traveling southbound on Wiswall Rd. from Dedham Street when he was attempting to pull into his driveway at 89 Wiswall Rd. WONG further states that as he attempted to turn Motor vehicle #2 passed him on the inside causing him to strike motor vehicle#2 on the drivers side. WONG stated to me that he used his right turning signal. Operator of motor vehicle#2, CAI Wei, states that he was traveling behind motor vehicle#1 and he thought that motor vehicle #1 was taking a left into 88 Wiswall Rd. He states that Motor vehicle #1 made movement to the left toward the center yellow double line as if he was turning left so he attempted to pass motor vehicle#1 when it suddenly turned right into 89 Wiswall Rd. causing him to strike the front passenger side of the motor vehicle. CAI states that motor vehicle #1 never used a turn signal.

The medics responded for an evaluation as a precaution even thought all parties stated they did not need medical attention. I spoke with Predina first who stated she was traveling northbound on Lewis St. when she entered the intersection of Newtonville ave she collided with VEH 2. Predina stated she did not see VEH 2 coming. I spoke with Gabrielle Sichel who stated she was traveling eastbound on Newtonville Ave when VEH 1 exited northbound from Lewis St. colliding with her. All parties were provided with an accident report number. All parties stated they were uninjured. Both vehicles were driven away from the scene.

On Wednesday, April 10th 2019, at approximately 5:31pm, I, Officer Brooks, responded to Crafts St street at Albemarle road, for a report of a 2 car MVA with injuries. Upon my arrival I spoke with the operator of MV1 (MA REG 61MS57), who stated he was traveling southbound on Albemarle road. He stated he stopped at the stop sign at the intersection with Crafts street, and then proceeded over Crafts street down Albemarle road. The operator stated he was struck by MV2 as he was crossing Crafts street. MV1 sustained heavy damage to the passenger side front end and wheel well area. The operator of MV1 was not injured. I then spoke with the operator of MV2(MA REG 11809), who stated she was traveling eastbound on Crafts street, when MV1 pulled out in front of her. She was not able to stop and her vehicle struck MV1 in the middle of the intersection. MV2 had damage to the front end. The operator of MV2 was complaining of pain in her head from striking the steering wheel. A witness to the accident stated he was stopped behind MV1 at the intersection of Albemarle road and Crafts street. He stated that MV1 accelerated across the intersection without yielding to MV2, and that there was no time for MV2 to stop safely. Both vehicles were towed due to the damage. The operator of MV1 was issued in hand MA Uniform Citation T1442386 for 89/9 Failure to safely proceed after stopping. The operator of MV2 was transported by Cataldo to NWH.

The weather at the time of the accident was clear and sunny. The road surface was dry. Washington Street is a public way maintained by the City of Newton. The operator of MV1, Jacqueline Dupre (S29256883), stated she was parked in front of 1302 Washington Street (E) in her 2018 Honda CRV (MA: 5054RF). Dupre stated she was attempting to exit her street parking spot and did not see any vehicles traveling towards her as she entered Washington Street. Dupre stated MV2's passenger side hit her front driver side area as she was attempting to enter the lane. I observed minor damage to the front driver side quarter panel/headlight area. Dupre reported no injuries at this time. The operator of MV2, Donnell Stoute (S53698210), stated he was operating a 2017 Porsche Macan (MA: 7ZC447) East on Washington Street. Stoute stated as he was traveling by 1302 Washington Street, MV1 pulled out of her parking spot and hit the passenger side of the vehicle he was operating. I observed moderate damage to the passenger side of MV2. Stoute reported no injuries at this time.

No injuries. #2 appeared to sustain moderate damage, was leaking fluids and required towing. Tody's towed #2 to their storage yard without incident. Witness was walking on the Chestnut St sidewalk. Witness stated he observed #1 edging out (stop and go fashion) into the intersection before he observed #1 fully proceed forward colliding with #2.

On May 3, 2019 at approximately 18:18 I responded to a motor vehicle crash on Beacon St at the intersection of Hammond Pond Pkwy. It was reported that a school bus was involved with no passengers on board. Upon arrival, both vehicles were pulled over on the westbound lane of Beacon St. Both vehicles were turning left onto Beacon St. from Hammond Pond Pkwy, which has two left turn only lanes. MV1 was in the left lane, and MV2 was in the right. MV1 attempted to merge with MV2 on Beacon St. but misjudged the turn and drove into MV2's driver's side. MV2 was a school bus contracted by the City of Newton with no passengers on board. Photographs were taken and submitted to IT to be attached to this report. No reported injuries.

Cedeno stated as she was traveling in the left turn lane on Centre Street, she attempted to travel straight (N) through the intersection at Washington Street towards 255 Washington Street. Cedeno stated the front passenger side of her vehicle crashed into the driver side of MV2. MV2 was attempted to continue left around the rotary and was located on the passenger side of her vehicle. Cedeno reported no injuries. I observed minor damage to the front passenger side tire area of MV1. Cendeno's passenger, Cristina Velez (S04276767), reported not injuries. The operator of MV2, Mary Sanker (S62890912), stated she was operating her 2016 BMW 328XI around the Washington Street/Centre Street rotary. Sanker stated as she was continuing through the rotary in her lane, the vehicle traveling in the lane to her left (MV2) entered her travel lane in an attempt to travel straight towards 255 Washington Street and crashed into the driver side of her vehicle. I observed moderate damage to the front and rear driver side doors of MV1. Sanker reported no injuries. The operator of MV1 was cited for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care)

The weather at the time of the accident was clear and sunny. The road surface was dry. Waverly Avenue and Franklin Street are both public ways maintained by the City of Newton. The operator of MV1, Dorothy Wensink (S40227061), stated she was operating her 2010 Toyota Prius (MA: 538EV6) Eastbound on Franklin Street towards Waverly Avenue. Wensink stated she came to a stop at the stop sign on Franklin Street (E) at Waverly Avenue. Wensink stated as she began to travel through the intersection, a school bus traveling on Waverly Avenue (S) crashed into the rear driver side door area of her vehicle. Wensink reported no injuries on scene. I observed moderate damage to the rear driver side area of MV1. Wensink was able to driver her vehicle away from the scene. MV2 was a City of Newton contracted yellow school bus (MA BUN: 39173) transporting students attending the Ward Elementary School. The bus was returning to the Ward School from a field trip. The operator of MV2, Antonio Goncalves (S54275856), stated he was operating the school bus Southbound on Waverly Avenue towards Franklin Street. Goncalves stated MV1 abruptly accelerated across the intersection from Franklin Street. Goncalves stated he was unable to avoid making contact with MV1 and crashed into the rear driver side of the vehicle. I observed minor damage to the front middle and passenger side bumper area of MV2. The following Ward Elementary School student, teachers, and parents were traveling on the school bus at the time of the accident and reported no injuries. Students: Avishai Arbili, Eleanor Chen, Maeve Donovan, Esat Efendigil, Mark Elentuck, Julian Ismay, Rhea Khanna, Griffin Kirby, Ori Layani, Priscilla Le-Ngo, Callum McLaughlin, Carolina Muniz, Emily Ney, Hrithik Ponduru, Levi Rosenberg Van Gameren, Zachary Schwarzberg, Ava Smoller, Matthew Somers, Lillian Welford, William Balerna, Alexandra Bogdanova, Randi Green, Christopher Iatrou Johnson, Charlotte Nichols, Henry Pozen, Sofia Shadah Alcalde, and Zoey Steinberg. Teachers: Katherine Goldman and Lauren Ludman. Parents: Enrique Shadah, Brian Rosenberg and Xinxin Wang. The Principal of the Ward Elementary School, Becca Brogadir, responded to the crash scene and signed a patient refusal for all of the students with Newton Medics. The students, teachers, and parents, then walked back to the Ward Elementary School with Principal Brogadir. The school bus was able to drive away from the scene. Once the students arrived at the Ward Elementary School, the nursing staff there evaluated each child before they could return to class. The school staff stated they would make notifications to the parents of the children involved in the accident. After speaking with all parties involved, the operator of MV1 was cited in hand for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Pictures were taken of both vehicles and submitted to the IT Bureau.

The OP. of MV#1 states he was travelling eastbound on Beacon Street. As he was entering the intersection of Chestnut Street, MV#2 on the oncoming lane suddenly made a left turn in front of his path causing a collision. He pulled over and couldn't find MV#2 anywhere around. He describes MV#2 as a black pick-up truck. The OP. of MV#1 is a dog walker and there were 6 dogs inside the vehicle during the accident. He states the dogs appear fine and the collision wasn't a large impact. Witness #1 states he was the first car stopped on Chestnut Street waiting for the traffic light to turn green. He witnessed MV#2 cut in front of MV#1 and cause a collision. MV#2 then continued driving down Chestnut Street southbound and didn't stop. When they were all stopped at a traffic light for Chestnut Street and Woodward Rd., Witness#1 took down the plate of MV#2 as L92280 and described it as a black pick-up truck with possible MA commercial plates. He describes that it has a spare tire on the bed of the truck leaning against the back window. MV#2 then accelerated away when the light turned green and turned into a unknown side street. The owner of the plate given is for Eric P. Rowe out of Norton, MA. His vehicle model does match the description of a black pick-up truck. I gave Norton P.D. a call at approx. 1800 hrs. A short while later, they called back and said the pick-up wasn't home but gave me a cell phone number. I was able to speak with Eric Rowe and he stated he was the OP. of MV#2. He stated he was on Beacon Street westbound making a left towards Chestnut Street southbound. There was a car making a turn in front of him and he saw MV#1 slow down, which is why he made the turn. A minor collision happened so he stopped to look at this truck and saw no damage. He didn't see MV#1 stopped anywhere so he drove away thinking there was no need to exchange paperwork for no damage to his truck. The OP. of MV#2 was advised of the proper procedure and that he will be getting a citation in the mail. An incident report was also generated.

On 07/16/19 at approximately 09:38 I responded to a two vehicle crash on Needham St. outside of #141. The Operator of Veh #1 stated she was turning left on Needham St when the South bound traffic stopped to left her turn. As she turned, Veh#2 drove South down the multi-turn lane and they collided nearly head on. Heavy damage and air bag deployment to Veh #1. The operator was checked out by Medics and signed a patient refusal. The Operator of Veh #2 stated she was driving down the multi-turn lane South on Needham St.. She was preparing to take a left onto Jaconnet St when Veh #1 pulled out of parking lot and collided with her vehicle. I observed moderate to heavy damage to Veh #2 and noted that the vehicle didn't have its directional on. The operator of Veh #2 complained to left leg pain and was transported to Newton-Wellesley Hospital By Ambulance #1. Both vehicles were towed by Tody's Towing. At the conclusion of my investigation it appears as if both vehicles were at fault during this crash; Veh#1 for not yielding to right of way and Veh #2 for traveling too soon in a turn lane, no directional and driving too fast for the heavy traffic conditions. Both operators were given verbal warnings and there will be no citations issued at this time.

On Thursday, August 8, 2019, while assigned to Traffic unit N525, I responded to the area of Centre Street and Mt. Ida Street for a report of a 3 car motor vehicle crash with entrapment. The weather at the time of the crash was clear and sunny. The road surface was dry. Centre Street and Mt. Ida Street are both public ways maintained by the City of Newton. On arrival, I was updated by Sgt. Chisholm that one of the vehicles (MV3) had a female operator that was entrapped as a result of the collision. She was freed from the vehicle by the Newton Fire Department and transported to Beth Israel Hospital. Another operator (MV2) was transported to Newton Wellesley Hospital with minor injuries. A third operator (MV1) remained on scene with no injuries. With the assistance of Officer Wade we conducted a survey of the crash scene. We observed a large debris field at the intersection of Centre Street and Mt. Ida Street with debris from all three vehicles. The point of impact was observed to be on Centre Street (N) before Mt. Ida Street. There were fresh scrapes in the pavement as a result of the colission. We also observed skid marks from MV1 measuring 57 feet 8 inches from the start of it's skid to the vehicles final rest MV1 crashed into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane of Centre Street and crash into MV3 as it was passing by. The impact from MV2's front end crashing into MV3's driver side resulted in the operator of MV3 becoming entrapped. MV2 and MV3 came to final rest on Wesley Street (W) at Centre Street (S). The driver door of MV3 was inaccessible. The collsion resulted in air bag deployment for MV2 and MV3. I observed moderate damage to MV1's front grill/bumper area. I observed substantial damage to the front and rear of MV2 and substantial damage to the front and driver side of MV3. Tody's responded and removed MV2 and MV3 from the roadway. Photos were taken of the scene and submitted to the IT Bureau. Officer Martin of the Watertown Police Department was requested by Sgt. Lee to respond to the scene and assist with a commercial truck inspection of MV1. Officer Martin contacted Trooper Grey of the Massachusetts State Police Truck Team who also responded to assist with the truck inspection. Sgt. Lee remained with Officer Martin and Trooper Grey during the inspection. MV1 was relocated to Richardson Street for the inspection. Trooper Grey determined the truck was in compliance with all required paperwork and it passed it's mechanical inspection. MV1 was removed from the roadway by a state tow. I spoke with the operator of MV1, William Tisbert (S24792655). Tisbert stated he was operating a 2006 KW W900 Semi Truck (NH SEMI: AP2638) Northbound on Centre Street. Tisbert stated as he was approaching Mt. Ida Street, a vehicle came out of no where and he hit it. Tisbert stated there was no one in front of him and then a vehicle came off of a side street (Mt. Ida Street) and attempted to turn left in front of him to return to the side street. Tisbert stated he could not avoid hitting the vehicle. Tisbert crashed into the rear of MV2. MV2 then crossed into the opposite lane and crashed into another vehicle. Tisbert remained on scene and reported no injuries. Tisbert stated he was wearing his seatbelt at the time of the accident and was not distracted. Tisbert later admitted to crashing into the rear of MV2. I spoke with the operator of MV2, Janet Vanbuskirk (S59829146), at Newton Wellesley Hospital. Vanbuskirk stated while operating her 2004 Toyota Highlander (MA: RS 529Y), she was stopped on Centre Street (N) at Mt. Ida Street. Vanbuskirk had her left directional on and was waiting for traffic in the Southbound lane to clear to take a left turn onto Mt. Ida Street. Vanbuskirk stated MV1 crashed into the rear of her vehicle. As a result of the impact, Vanbuskirk's vehicle traveled into the opposite lane and crashed into the driver side of MV3. Vanbuskirk reported minor injuries to her neck, left arm, and right hand. I spoke with the operator of MV3, Madeleine Karolian (S23979811), at Beth Israel Deaconess Medical Center in Boston. Karolian stated she was operating her 2016 Toyota Prius (MA: EV5746) Southbound on Centre Street. Karolian stated MV1 crashed into MV2, and MV2 crashed into her vehicle. Karolian stated she did not have time to avoid the collision. Karolian stated she had minor injuries to her neck and chest. Karolian's vehicle had a dash cam recording at the time of the accident. I viewed the footage with her through an app on a cell phone. The footage shows MV3 traveling Southbound on Centre Street on August 8, 2019 at 1308 hours. Ahead of MV3 in the Northbound lane, MV2 is seen stopped on Centre Street before Mt. Ida Street with it's left directional activated. I observed MV1 travel Northbound on Centre Street and crash into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane and crashed into the driver side of MV3. The footage then ends. A copy of the footage was emailed to my work email. The footage was then forwarded to IT Director Steve Smith to be added to the media for this report. As a result of my investigation, the operator of MV1, William Tisbert, will be cited for Newton City Ordinance Chapter 19, Section 71 (Following too closely). Massachusetts Uniform Citation T1270942 was completed and mailed to Tisbert's residence.

Opr of V1 stated V2 drove through the red light. Opr of V1 stated V2 took off on Centre Street towards Watertown, MA. V1 was towed by Tody's. Dispatch was notified to contact Watertown PD. Watertown PD found V2 in Watertown at the corner of Warren and Endenfield. Officer Brandon spoke to the Opr of V2. V2 stated she was nervous so she left the area. Officer Brandon advised her she needs to stay on location next time. Officer Brandon gave the Opr of V2 a copy of the criminal application (T2014370). The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave making a left turn onto Morton St when he was struck by MV#2. The operator of MV#1 stated he was unsure of where MV#2 came from. There was a strong solar glare facing westbound on Commonwealth Ave during this accident. MV#1 sustained heavy driver's side front end damages. Both front airbags were deployed. The operator of MV#1 stated he was not injured. The front passenger of MV#1 was not injured and had already left the scene to attend school. The driver's side rear passenger sustained a bleeding nose from this accident. She was unsure what caused her injuries because it happened too quickly. She was evluated by Newton Paramedics and signed a patient refusal of treatment. The initial point of impact for both vehicles was in the middle intersection of Commonwealth Ave at Morton St. There were broken vehicle parts and tire skid marks starting from the point of impact and onto the berm on Commonwealth Ave on the carriage lane side. MV#2's final resting spot was directly accross from 808 Commonwealth Ave. MV#2 sustained heavy front end damages to its driver's side wheel (Broken axle) and flat front right side tire. MV#2's driver's side airbag was deployed. The operator of MV#2 was transported by Newton Paramedics to the Boston Medical Center. I spoke to the operator of MV#2 via telephone and she stated she was travelling westbound on Commonwealth Ave when she was struck by MV#1 and caused her swerve onto the carriage lane berm. The operator of MV#1 is 6.5 months into her pregnancy and sustained neck, back, bruises and left hand injuries. She was transported to Boston Medical Center and was later discharged the same day. She was provided with this accident report number and was advised to contact her insurance provider. I spoke to the witness who was at the intersection of Cedar St and Commonwealth Ave. The witness stated MV#1 was travelling eastbound on Commonwealth Ave and had stopped, attempting to make a left turn onto Morton St. The witness stated she then observed MV#2 travelling westbound on Commonwealth Ave and struck MV#1. The witness stated she believed MV#2 was travelling at a high rate of speed because it continued to travel onto the berm after striking MV#1. I took digital photographs of the damaged berm on Commonwealth Ave. Based on the statements made to me, I issued in hand to the operator of MV#1 citation #T1268357 for violation of c89 s8 failure to yield when turning left. Both vehicles were towed by "Tody's Towing".

On Saturday, September 28, 2019, I responded to the intersection of Crafts Street and Albemarle Road, Newton with Sgt. Lee for a report of a motor vehicle crash involving a motorcycle with serious injury to it's operator. The weather at the time of the crash was clear and sunny. The road surface was dry. Crafts Street and Albemarle Road are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jussara Pontes (S92930243). Pontes stated she was operating a 2017 Toyota Sienna (MA: 4GX777) at the time of the crash. Pontes stated she was traveling North on Albemarle Road and came to a stop at the stop sign located at the Crafts Street intersection. Pontes stated she looked both ways and began to inch out into the roadway. Pontes stated she stopped the vehicle again and was assisting a child sitting in the 2nd row of the vehicle. Pontes stated she then turned her attention back to the roadway. Pontes stated she looked to her right and left again and did not see any vehicles in either direction. Pontes stated she proceeded to cross the intersection to continue North on Albemarle Road. Pontes stated the front bumper area of MV1 crashed into the left side of a passing motorcycle. Pontes stated the motorcycle and it's operator fell to the roadway and slid across the street to the side of the road. Pontes stated she pulled her vehicle over onto Albemarle Road. Pontes stated she did not see the motorcycle prior to the crash and was not distracted. The operator of the motorcycle (MV2), Irene Pistoftzian (S06099449) was transported to Brigham and Women's Hospital with a serious left leg injury prior to my arrival. I observed a red 1990 Kawasaki ZR550 (MA MCN: 1V1063) resting on it's right side on the NE sidewalk area of Albemarle Road at Crafts Street with it's front wheel/handlebar facing Crafts Street. The motorcycle had moderate damage to it's left side from the impact of MV1 crashing into it. The right side of the motorcycle had damage from sliding on the roadway and the crashing into a fence on the sidewalk on Crafts Street (W) at Albemarle Road. There was also damamge to the front headlight/right handlebar area. The motorcycle and Toyota Sienna were towed from the scene by Tody's towing at Sgt. Lee's request. Pictures were taken of the scene and submitted to the IT Bureau. I spoke with Pistoftzian at Brigham and Women's Hospital in Boston on Monday, September 30, 2019. Pistoftzian stated she was operating her motorcycle on Crafts Street (W). Pistoftzian stated as she approached the intersection of Crafts Street and Albemarle Road, she observed a mini van to her left out of her peripheral. Pistoftzian stated she remembers being hit on her left side and then laying on the roadway looking up at the sky on the sidewalk with her head facing Albemarle Field. Pistoftzian stated as a result of the crash she has a significant injury to her left leg and also has an injury to her left wrist/arm. Pistoftzian stated she was wearing a helmet at the time of the crash. I spoke with Kymberley Mitchell via telephone who stated she witnessed the crash. Mitchell stated she was traveling behind the motorcycle involved in the crash on Craft Street (W) at North Street. Mitchell stated they were stopped at a red light at this time. Mitchell stated there were no vehicles in front of the motorcycle at the light. Mitchell stated the light turned green and they continued on Crafts Street at a low rate of speed. Mitchell stated she could see ahead at the intersection of Albemarle Street and Crafts Street, traffic was stopped on Crafts Street (E) letting traffic exit Albemarle Road (N). Mitchell stated vehicles were taking left and right turns out of Albemarle Road before the motorcycle reached the intersection. Mitchell stated a mini van was crossing past the double yellow line of Crafts Street to continue straight when it crashed into the left side of the motorcycle. I spoke with Cheryl Wermer via telephone who stated she witnessed the crash. Wermer stated she was traveling on Crafts Street (E) towards Albemarle Road. Wermer stated traffic ahead of her was stopped to allow vehicles to turn off of Albemarle Road. Wermer stated she observed a motorcycle traveling towards her on Crafts Street (W). Wermer stated she then saw a vehicle travel straight from Albemarle Road across Crafts Street and crash into the motorcycle. Wermer stated the operator of the motorcycle flew off the bike. Wermer stated the car then came to a stop and the driver got out and stated "Oh my god! I didn't see you!" Wermer stated both vehicles were traveling at a low rate of speed prior to the crash. I spoke with Bill Frabizio via telephone who stated he witnessed the crash. Frabizio stated he was behind the mini van (MV1) on Albemarle Road prior to the crash. Frabizio stated the vehicle in front of him began to cross the intersection as he was monitoring the traffic on to take a right turn onto Crafts Street. Frabizio stated he heard a loud "Boom" and saw a motorcycle slide across the street with its operator. Frabizio stated the motorcycle crashed into a fence post and the operator came off the motorcycle. Frabizio stated he pulled over and assisted operator of the motorcycle. After concluding my investigation, the operator of MV1, Jussara Pontes, was mailed Massachusetts Uniform Citation T2016424 for Newton City Ordinance Chapter 19, Section 75 (Failure To Use Care).

SHE STATES SHE NEVER SAW THE MOTORCYCLE UNTIL IT WAS HITTING HER VEHICLE. OPERATOR OF VEHICLE #1 WAS BEING ATTENDED TO BY THE FIRE AND MEDICS. OPERATOR OF VEHICLE #2 WAS GIVEN MOTOR VEHICLE CITATION ( T1445177, 89/9, FAILURE TO YIELD AT INTERSECTION ). OPERATOR SERVED CITATION IN HAND. I SPOKE TO A WITNESS ( TIMOTHY CHANGOGARRETT ). HE STATED HE WAS TRAVELING E-BOUND BEHIND THE MOTORCYCLE. HE SAW VEHICLE #2 ENTER THE TRAFFIC LANE AND VEHICLE #1 HIT HER VEHICLE. HE STATES THE OPERATOR OF VEHICLE #1 WENT OVER HIS HANDLE BARS UPON COLLISION. LANDED ON THE HOOD OF VEHICLE #2 AND THEN FELL TO THE GROUND. AT THIS POINT HE CALLED 911. OPERATOR OF VEHICLE #1 WAS TRANSPORTED TO BRIGHAM AND WOMEN'S HOSPITAL BY MEDIC2 FOR FURTHER TREATMENT. I WENT TO THE HOSPITAL AND SPOKE TO THE OPERATOR IN HIS ROOM. HE STATES HE WAS TRAVELING E-BOUND ON CRAFT INTENDING TO GO TO THE POST 440 ON CALIFORNIA ST. TO ATTEND AN AA ( ALCOHOLICS ANONYMOUS ) MEETING. HE STATES HE TOOK HIS SON'S MOTORCYCLE WITH THE INTENTION OF SEEING IF THERE WERE ANY REPAIRS NECESSARY BECAUSE HIS SON IS TRYING TO SELL IT. WHILE TRAVELING HE APPROACHED THE INTERSECTION. HE SAW VEHICLE #2 ENTER ONTO CRAFT ST. HE STATES HE COULD NOT DIVERT AND HE WAS UNABLE TO AVOID HITTING HER VEHICLE. OPERATOR HAS BROKEN BONES IN HIS LEFT ARM. CUTS ALONG HIS FACE AND NECK AS WELL AS LOWER BODY PAIN. HE STATED THE HOSPITAL INTENDS ON KEEPING HIM OVER NIGHT FOR OBSERVATION. I ASKED THE DRIVER IF HE WAS AWARE THAT HIS LICENSE TO OPERATE A MOTOR VEHICLE WAS REVOKED. HE STATED HE HAD A CHANCE TO BE REINSTATED BUT CHOSE NOT TO. DRIVER WAS ISSUED MOTOR VEHICLE CITATION ( T1445178, 90/23/C, LICENSE REVOKED AS HTO ). I INFORMED THE OPERATOR THAT I WOULD MAIL HIM THE CRIMINAL APPLICATION. ALL OTHER PARTIES REPORTED NO INJURIES. BOTH VEHICLES WERE TOWED BY TODYS. VEHICLE #1 HAD FRONT END DAMAGE AND FRAME DAMAGE. VEHICLE #2 HAD LEFT SIDE DAMAGE AND A BROKEN AXLE. WEST NEWTON SQUARE ( OFF L. MIKOLEIT ) ARRIVED ON SCENE AND TOOK 10 PICTURES. PICTURES PLACED IN I.T. BOX. I CLEARED THE AREA WITHOUT FURTHER INCIDENT.

On 10/04/19 at 11:49 hours while assigned to Traffic Unit 523, I responded to a motor vehicle/bicyclist accident at Centre Street and Nathan Road. The weather at the time was cloudy. The road surface was dry and Centre Street and Nathan Road are public ways maintained by the City of Newton. Traffic conditions were heavy and Centre Street was congested with vehicles at the time. There is no dedicated bicycle lane in this area but there are bicycle indicators painted on the roadway to make drivers aware of bicyclists traveling. I spoke to the bicyclist involved (Unit 2), Marvin Goldman (S67373495). Goldman was wearing a helmet, gray sweatshirt, gray shorts, and black/orange reflective sneakers. Mr. Goldman stated that he was traveling southbound on Centre Street on the right side of the road on his black Fuji Sagres bicycle. He further says that he was peddling fast at the time while crossing over Nathan Road and that is when he suddenly struck Unit 1 on it's passenger side front quarter panel/door area. He does not recall any other details of the incident along with Unit 1 attempting to turn left towards him. Goldman sustained scraped up knees and a small laceration below his left eye. He refused medical attention signing a patient refusal with Cataldo Medic One. I observed that his front wheel was bent as a result of the accident. The rest of the bicycle appeared to be intact. I then spoke to the operator of Unit 1, Darlene Martins (S27966454). Ms. Martins spoke with the assistance of Ofc. Calderon who was translating for her. She says that she was traveling northbound on Centre Street and was attempting to turn left on to Nathan Road in order to turn around and travel the other way on Centre Street. She says that a vehicle in the southbound travel lane left space for her to make her left turn. She then proceeded to turn thinking it was clear and that is when the bicyclist suddenly approached and struck her on her passenger side. She also stated that it was hard to see past the congestion of vehicles waiting in the southbound travel lane. I observed scratch marks and a dent to her passenger side front quarter panel area along with minor scuff marks to her front passenger door. The scuff marks appear to be from the bicyclist rubbing against the vehicle. I also observed a dent on the top of the passenger door frame. It is unknown if the damage is the result of the accident or old damage Ofc. Calderon spoke to Rosangela Dias (Passenger in Unit 1), who stated that everything happened so quick but she never saw the bicyclist approaching when Martins attempted to turn left. I then spoke to a witness on scene identified as Matthew Miner. He says that he was two vehicles behind an unknown vehicle in the southbound travel lane of Centre Street. He says that the unknown vehicle left space for Unit 1 to turn left and waved Unit 1 on to turn left. He does not recall what transpired with the bicyclist. As a result of the accident, it was determined that Martins did not yield the right of way to the bicyclist while she was turning left. She also does not have a license to operate in Massachusetts or any other state. She was issued MA Uniform Citation # T2012393 for Ch. 89/8 Failure to yield and Ch. 90/10 Unlicensed Operation of a Motor Vehicle. Digital photos taken by me of the scene.

Operator of vehicle 1 stated he was traveling eastbound on River St and was taking a left turn onto Cherry St and was struck head on with vehicle 2. Operator of vehicle 2 stated she was traveling westbound on River St and as she was entering the intersection of Cherry St the traffic light turned yellow and vehicle 1 which was traveling eastbound made a sudden left turn leaving her no time to stop and the vehicles collided. Vehicle 1 then sped off northbound on Cherry St and returned a minute or two later. I spoke to a witness, Jena Castro-Casbon who stated she saw vehicle 1 traveling northbound on Cherry St at a high rate of speed and drop of a child at 187 Cherry St then turn around and return to the accident scene at a high rate of speed again. 187 Cherry St is 0.4 miles from the intersection of River St. Operator of vehicle 1 stated he had two of his sons friends in the vehicle and he thought it would be best to get one of them home to 187 Cherry St and then come back to exchange information. I followed up with the child and his parents at 187 Cherry St and he was uninjured. I issued the operator of vehicle 1 MA Uniform Citation #T1445381 for 89/8 failure to yield. No one reported any injures and neither vehicle required a tow.

As MV#2 turned eastbound it collided with MV #1. Significant damage was observed to both MV#1 and MV#2's front bumpers and engine compartments. Both MV#1 and MV#2's airbags were deployed upon impact. The Operator of MV#1 reported she was uninjured while the Operator of MV#2 was transported to the hospital. Both vehicles were towed from the scene by Tody's Tow Service.

Both streets are public ways and maintained by the City of Newton. Operator of M/V # 1 states he was traveling straight ahead (EB) on California Street through the intersection at Bridge Street. He states that he had the green light signal and attempted to drive through when he was struck by M/V # 2 on his front bumper on the driver's side. He further says that M/V # 2 was attempting to take a left turn on to Bridge Street at the time of the accident. I observed moderate damage to the driver's side front bumper/quarter panel area but the vehicle was still driveable. Operator of M/V # 2 states through translation by her husband (Huseyn Imamaliyev) she was operating (WB) on California Street and was attempting to take a left turn through the intersection on to Bridge Street. She says that she stopped first and then attempted to turn left and that is when M/V# 1 crashed in to her. I observed minor damage to the middle front bumper/registration plate area. Based on the locations and damage to the vehicles, it appears that the operator of M/V # 2 did not yield the right of way when turning left on to Bridge Street. The operator of M/V # 2 (Gulnar Imamaliyeva MA ID# SA1320134) also could not provide a driver's license to operate a motor vehicle in Massachusetts. A query of her revealed that she only had a Massachusetts identification. She was issued MA Uniform Citation # T2012157 for Ch. 89/8 Fail to Yield Right of Way When Turning Left and Ch. 90/10 Operating a M/V Without a License. HORN states Motor Vehicle #2 was taking a left onto Vine Street from Brookline Street and turned to sharp striking the front drivers side of his vehicle. HORN states that he was not moving as he was waiting for traffic to clear. After speaking with HORN I spoke to the operator of Motor Vehicle #2, QIAN Xiaohua, who states that she was turning left from Brookline Street onto Vine Street. She states while turning left she didnt see Vehicle #1 waiting to turn and she struck the front of his vehile causing damage to the drivers side door of her vehicle. After hearing from both parties involved and observing the damage to both vehicles it appears to me that the operator of MV#2 (QIAN) violated the right of way of MV #1 (HORN). The statement by HORN that he was not moving and was simply waiting to turn is consistent with the damage to the vehicles. Had HORN been moving the damage would have occurred to the Passengers side of his vehicle and the front drivers side of vehicle#2 (QIAN). The statements made by both operators and my observations lead me to believe that the operator of MV#2 (QIAN) is at fault for this Crash

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

Vehicle one, Newton police marked 503 cruiser operated by Officer Hagopian, was stopped behind a stopped unknown vehicle on Watertown Street facing West waiting on a red light. Officer Hagopian stated both vehicles were in the right travel lane. Vehicle one was the second car in line at the red light. Officer Hagopian of vehicle one heard a police broadcast of a victim in cardiac arrest and activated its police emergency lights and siren to respond to the emergency call. The unknown vehicle in front of vehicle one pulled to the right and allowed vehicle one to navigate around it. Officer Hagopian stated he had his emergency lights and siren activated and went around the stopped vehicle that had pulled to the right in front of him. Vehicle one proceeded West toward the intersection at a slow rate of speed and observed the intersection to be clear and then accelerated. Officer Hagopian stated that vehicle two came from Walnut Street, traveling North, and came across the intersection where vehicle one and two collided. Officer Hagopian estimated his cruiser to be traveling at 15-20 mph upon impact with vehicle two. The front end of vehicle one made impact with the passenger side of vehicle two. Vehicle two, BMW operated by Gail Glaser, was traveling North on Walnut Street and was approaching the intersection of Walnut Street and Watertown Street. Operator of vehicle two estimated her vehicle speed to be about 20 mph as her vehicle entered the intersection. Ms. Glaser stated she had a green light. Operator of vehicle two was in the middle travel lane that was a designated lane to go straight. Mrs. Glaser stated she did not see an emergency vehicle but could hear the siren as she approached and then entered the intersection from Walnut Street (going North). She stated she had a clear view of the intersection. Vehicle two then came in contact with vehicle one (going West on Watertown Street) in the intersection. Vehicle two then came to a final rest in the Southbound lane of Walnut Street near the traffic signal posts at the corner. Vehicle two made head on contact with vehicle three upon final rest. Vehicle three, Acura operated by Jeffrey Katz, was the first car in the right travel lane on Walnut Street facing South. Mr. Katz stated he, vehicle three, was stopped at a red light that had just turned to a green light. Mr. Katz stated he heard the siren, stay stopped, and looked towards the direction of the sound and saw a marked cruiser entering the intersection from Watertown Street (West). Mr. Katz remained stopped where he was and then saw vehicles one and two collide in the intersection. Vehicle two then veered into the front of his vehicle head on and contact was made. Mr. Katz stated when he initially saw the cruiser it was already in the intersection with emergency lights and siren activated and estimated the cruiser to be going about 20 mph upon impact. Mr. Katz also indicated that vehicle two was traveling into the intersection from Walnut Street (North) at an estimated 20 mph. Witness Peter Dill was sitting in his parked car on Watertown Street facing East. Mr. Dill was parked in front of his house at 606 Watertown Street. It is the second home in from the intersection on the Eastbound travel side of Watertown Street. Mr. Dill Stated that he was looking at his cell phone when he heard the siren. When he looked up towards the sound of the siren, he saw vehicle one traveling about 5 mph in the intersection with emergency lights and siren activated. He did not know where vehicle one had originated from and just that it was traveling West on Watertown Street. Mr. Dill also saw vehicle two approaching from Walnut Street heading North and was not speeding. Mr. Dill estimated that 3-5 seconds had gone by when he first heard the siren to when the initial impact of vehicles one and two occurred. Witness James Davis-Sanon was driving a large MWRA utility work truck. He was the first car in the right turn only lane on Walnut Street. He stated that Walnut Street had the green light and he stopped before the intersection to allow vehicle one to continue going West on Watertown Street. Mr. Davis-Sanon stated he saw/heard vehicle one activate its emergency lights and siren when vehicle one was stopped behind a car waiting on a red light on Watertown Street. After vehicle one activated the emergency lights and sirens, it went around the car stopped in front of it and proceeded into the intersection at a slow rate of speed that Mr. Davis-Sanon described as a "rolling stop" but could not remember if the cruiser stopped or nearly stopped. Mr. Davis-Sanon stated vehicle one then accelerated to about 15 mph into the intersection. Mr. Davis-Sanon also looked at his driver's door rear view mirror and saw vehicle two traveling North on Walnut Street in the designated straight only lane and then proceed into the intersection. He estimated that vehicle two was traveling at 25 mph upon impact with vehicle one. Witness Joseph Giangregorio was in the front passenger seat of the same MWRA utility work truck. Mr. Giangregorio concurred with Mr. Davis-Sonon witness accounts regarding the location of their truck and vehicle one prior to emergency lights and sirens. Mr. Giangregorio did not witness the rear view mirror account since Mr. Giangregorio did not observe vehicle two in the rear view mirror. Mr. Giangregorio added that vehicle one activated its emergency lights and siren, maneuvered around the stopped car in front of it, slowed to 1-2 mph before the intersection but could not remember if it was stopped and then accelerated to about 15 mph before impact with vehicle two. He estimated that vehicle two was traveling 25 mph upon impact with vehicle one. I canvassed the area of where this incident ocurred for cameras. The only cameras I was able to locate was at 575 Watertown Street which is the second house in from the intersection on the Westbound travel side. Both cameras at this address did not have an angle to capture the intersection. Both cameras are focused on the front door to the home and the homeowner, Jose, confirmed that the angle could not capture the intersection. Operator of vehicle two was cited for failure to yield to an emergency vehicle, MA uniform citation T2012104. Citation was written on February 3, 2020 and sent via mail.

On 02/08/20 I responded to the intersection of Winchester St and Curtis St for a two vehicle crash. M.V. #1 stated she was driving due West on Winchester St. when veh #2 side swiped her in the same direction. The Operator of veh#2 stated he was driving due West on Winchester St. when veh #1 tried making a U-turn near Curtis St. and turned into his passenger side. The operator of veh#2 does not have an active license and was driving a work van with an expired registration. Both vehicles were towed by Tody's Towing. The operator of veh#2 was handed Mass. Criminal Citation #T2013434 for Ch 90 / Sec 23- Operating after License Suspension (subsequent offense) and Ch 90 / Sec 9- Unregistered M.V. on a public way. Operator of veh #2 has 11 arraignments on his adult B.O.P. with mostly motor vehicle offenses. I completed a towed m.v. form for veh #2, scanned it into this report and placed the citation into the court packet. All roads traveled/mentioned in this report are public ways in the City of Newton as they are serviced and maintained by this city.

On March 16th, 2020 at approximately 09:38 hours while working N491 I responded to a report of two car MV crash at the intersection of Newtonville Ave at Lewis Terrace. On my arrival I observed a large while box truck(vehicle1 ) with passenger side front end damage parked on Newtonville Ave just west of Lewis Terr. The other involved vehicle(#2) was located inside the construction site at #255 Newtonville Ave. I spoke with the operator of the Truck(veh#1) who reported that he was driving W/B on Newtonville Ave when vehicle #2 pulled out into the intersection from north side of Lewis Terr striking the front passenger side of his truck. The impact from the crash caused vehicle #2 off the road, through a chain link fence and into the construction sit to #255 Newtonville Ave before coming to a stop. There were numerous construction worker on the job site but miraculously none of them were struck by vehicle #2 as it crashed through the fence onto the site. I later spoke with the operator of vehicle #2 who was not injured. he stated he was driving S/B on Lewis Terr. and was entering onto Newtonville Ave when he was struck by vehicle #1. Operator couldn't remember if he looked for oncoming traffic prior to entering the intersection. I spoke with the son of Mr Goldfinger(operator #2) who had arrived on scene and he assured me his father would no longer be driving again. I cited Mr Goldfinger for failing to yield/stop(c89/c9) at an intersection. Ticket # t1444075 Todys towed vehicle#2 from scene and there were no reported injuries. Witness stated she was behind vehicle #2 on Lewis Terr.and observed him slowly enter out into the intersection without coming to a complete stop. Pictures taken by Construction foreman. All insurance information and drivers info from operator #2 was taken by him also.

On 03/18/20 at approximately 13:01 I responded to the intersection of Winchester St and Heatherland Rd. for a two vehicle crash. Upon arrival I spoke with Newton Police Detail Officer Schaefer who was involved in the accident. Officer Schaefer was operating his own vehicle (Mass Reg#1JP123 Blue 2001 Cadillac Deville) and headed back to a paid Feeney Brothers detail. Officer Schaefer stated he was traveling due South down Winchester St. approximately 30mph when a light colored vehicle (later determined to be Mass Reg#9FG942- Grey 2010 Toyota Prius operated by Rosana Cristina Brasileiro) pulled out of the one-way Heatherland Rd, the wrong way. Officer Schaefer stated the Prius struck him on his drivers side front and rear doors causing the vehicle to crash into the curb and causing his side air bag to deploy. Officer Schaefer was wearing his seatbelt but struck his head off the interior of his vehicle. I attempted to speak with the operator of the other vehicle Rosana Brasileiro but she only spoke Portuguese. Newton Fire Department Engine #7 was also on scene and Fire Fighter Ian Moniz stated he spoke a different dialect but was able to communicate with her. Fire Fighter Moniz translated for me and I was able to ascertain that Brasileiro stated she was driving due West on Heatherland Rd(wrong way) when she didn't see the other vehicle and made contact with it. I asked Brasileiro for a License and she handed me a Brazilian Passport and stated she didn't have a license. She stated she was just visiting from Brazil and that she was operating her bosses vehicle. Brasileiro was handed Mass. Criminal Citation #T2080843 for Ch90/ Sec10- Unlicensed Operation, Ch90/ Sec 16-Operating on a one way the wrong way and Ch 89/ Sec 8-Failure to yield to the right of way. Both vehicles were towed from the scene while Officer Schaefer went with the Medics to Newton-Wellesley Hospital to get checked out. Sgt. McLean and Traffic Officer Wilson (N523) were also on scene and Officer Wilson took pictures. Sgt. McLean traveled to N.W.H. to check on Officer Schaefer. All roads mentioned/traveled on in this report are public ways in the City of Newton as they are serviced and maintained by this city.

On Thursday March 26, 2020 at approximately 1547 hours I was dispatched to the intersection of Commonwealth Ave at Lexington St (both public ways) for a motor vehicle accident. MV1 operator was unsteady on his feet, bloodshot and glassy eyes, slurred speech, and had an odor of alcoholic beverages coming from his breath. MV1 operator stated he was driving westbound on Commonwealth Ave and then took a left onto Lexington St southbound. MV1 operator stated he was then struck on the front passenger bumper by MV2. MV1 operator at the time reported no injuries. MV1 sustained heavy front end damage to the front passenger wheel well and was towed by Todys. MV2 operator stated he was driving eastbound on Commonwealth Ave and driving through the intersection because he had a green light. MV2 operator stated MV1 took a quick left hand turn in front of him and that he was unable to stop. MV2 struck the front passenger bumper of MV1 and MV2 sustained front end damage. MV2 operator reported no injuries. MV1 operator was placed under arrest for Operating after Suspension, OUI Liquor, Negligent Operation, and for a warrant out of Waltham District Court.
