Vehicle #1 stated she was traveling south bound on Grant Ave. Vehicle #1 stated that in the area where Grant Ave. bends to the left the roadway is dark and she had trouble seeing the road. Vehicle #1 stated at the last second she saw the road bending left and attempted to turn but skid on the snow and struck a small boulder in the front lawn of 173 Grant Ave. moving the rock a few feet. Vehicle #1 then skidded and came to a stop in the middle of the lawn of 173 Grant Ave. Vehicle # 1 then pulled off the lawn and to the side of the road. The owner of the property of 173 Grant Ave. was also present and was notified of the accident. Vehicle #1 had moderate damage to the front of the vehicle but was still able to be driven and was driven home. Both the operator and passenger in Vehicle #1 stated they were uninjured in the accident and refused any medical treatment.

On 2/1/19 I was working N496 when I responded to a 2 car MVA at 15 Cypress St. with a road rage incident ( incident # 19004330). Upon arrival I spoke with Edward Macaleese. Macaleese stated MA reg 2EE232 (being operated by Fabian Castillo) was tailgating him while traveling southbound on Cypress St. so he pulled to the side to allow him to pass. After Castillo passed Macaleese he slammed on his brakes causing Macaleese to rear end Castillo. I spoke with Castillo who stated that he was traveling southbound on Cypress St. when the vehicle in front of him (Macaleese) began driving "much slower than the speed limit". Castillo stated the Macaleese pulled over to allow him to pass, after Castillo began to slow in traffic due to a vehicle pulling out. Macaleese struck Castillo from behind. Both parties stated they were not injured. Both vehicle were still drive able. Both parties were provided an accident report number.

MV1 stated he was "driving to fast around the corner and hit the pole." MV1 actually hit a curb, took down a street light, went up on the island hitting another street light prior to hitting the curb. The second light he struck fell on MV2. MV1 sustained heavy front end damage and was removed from the scene by Tody's Towing. MV2 stated that MV1 "came flying across the intersection, crashed into 2 poles, one of which hit my car and he crashed over there." I then observed minor damage to MV2, however it did not require a tow. Both parties signed patient refusals with Cataldo Ambulance. Operator of MV1 was arrested and charged with the following: M.G.L. 90/24/J OUI Liquor M.G.L. 90//24/E Negligent Operation M.G.L. 89/4A Marked Lanes Photos of the scene were captured and turned over to the IT Bureau. Daigle Electric notified of the damaged lights.

MV1 was coming down the Rte 9 West off ramp to Chestnut St. Operator was traveling too fast for the snowy road conditions and failed to stop for the stop sign at the bottom of the ramp. A snow storm had started 2 hours prior to the accident and road conditions were slippery. MV1 struck MV2 broadside who was traveling north on Chestnut St with the right of way. MV1 struck MV2 hard enough that MV2 was pushed across the lane and into MV3 that was stopped at the stop sign southbound. According to the operator of MV2, MV1 was traveling "way too fast for conditions" over 30mph. Operator MV1 was cited for 89/9 failed to stop for stop sign and 90/17 speeding too fast for road conditions. Operator was also cited on Citation T1272992 for 90/23 operating after suspended license. Boylston St and Chestnut St are public ways in the city of Newton.

On Thursday 2/21/19 at approximately 0653 while assigned to marked unit N499 I was travelling westbound on Nahanton Street when I observed a motor vehicle pulled over to the right side of the road with its emergency flashers on. I pulled behind the vehicle and spoke with the operator, FINCH Robert L, who states that he was travelling Westbound on Nahanton Street and due to the weather conditions he lost control of his vehicle and slid into utility pole #14. There was very minimal damage done to the utility pole, however FINCH's car was disabled due to the collision. The weather conditions were poor as it was raining and there were several inches of snow on the ground from a storm that morning causing the roads to be slushy and slippery. FINCH was uninjured and stated that he had AAA on the way to tow his motor vehicle.

On February 27, 2019 at approximately 06:33 hours I responded to the area of 1091 Centre St for a motor vehicle crash with minor injuries. Upon arrival, fire and medics were evaluating all parties involved. The operator of MV2 complained of neck and back pain but refused medical transport but will be seeking care if pain continues. All motor vehicles were traveling Northbound on Centre St. slowing in traffic. MV1 crashed into the rear MV2, which in turn made MV2 crash into rear of MV3. MV1 admitted fault, claiming that she hit the gas pedal instead of the brake. MV2 was towed by the scene by Tody's.

On 3/4/19 I responded to the area of Hammond St and Chestnut Hill Rd for a report of a two car MVA. Upon arrival it was determined there were no injuries and the two drivers signed patient refusals with the ambulance. I first spoke with the operator of MV#1 who stated he was driving northbound on Hammond St near the Chestnut Hill T station where it bends and MV#2 was coming southbound and skidded on the snow and came into his lane. I then spoke with the operator of MV#2 who stated he was driving southbound on Hammond St when his vehicle lost control around the turn and drifted into the opposite lane and struck MV#1. I issued the operator of MV#2 MA Citation T1268628 for Chapter 89/4A; Marked Lanes Violation. At the time of this accident there were heavy snow conditions. Tody's Towing responded and towed both vehicles from the scene.

On the above date/time I responded to the area of Cherry Street and Webster Street for a three motor vehicle crash with injuries. Upon arrival along with Sgt. Dempsey, Fire/medics. I spoke with the operator of vehicle one. He stated he was stopped on Webster Street facing westbound a the red traffic signal. He further stated the traffic signal on Webster Street turned green, he accelerated forward. He stated vehicle two traveled north on Cherry Street at a high rate of speed through the red traffic signal. He stated he had no reaction time due to the speed of vehicle two and had impact. I spoke to the operator of vehicle two. He stated he was traveling north on Cherry Street going too fast, he stated he thought it was a school zone so he slowed down and he did not observe the color of the traffic signal. He stated he observed vehicle one come out of the Webster Street at the last second at impact. I spoke with the operator of vehicle three. She stated only remembered being stopped on Cherry Street facing south. She stated she did not remember what color the traffic signal was, just knew she was stopped when she felt heavy impact from vehicle two. Injuries: Oper of V1 none at this time- patient refusal Oper of V2 none at this time - patient refusal Oper of V3 Head laceration and head trauma, transported to Newton- Wellesley Hospital Tody's towing arrived on scene and towed vehicles two, three to their tow lot. All proper paper work was done for a towed vehicle(s). All parties advised of the process. After the vehicles were towed, and the scene was cleared I watched the cycling of the traffic signals in the intersection and observed they were functioning properly. It appears a this time based on statements and physical characteristics of the impact area on the roadway, that vehicle two went through the red light on Cherry Street, which caused the crash.

Mrs. Weiss stated that she was attempting to back out of her driveway at 285 Crafts Street when her vehicle accelerated for no reason "like a bat out of hell." The vehicle then crossed Crafts street at a high rate of speed, jumped the curb, crossed the large grass island, and only stopped when the vehicle crashed into a tree. Mrs. Weiss was uninjured and signed a patient refusal. The vehicle sustained moderate damage to the tires, undercarriage, and rear bumper and was towed from the scene by Todys. Photos were taken of the damaged city tree and were placed in the IT bureaus mailbox to be attached to this report. Based on the facts that Mrs. Weiss could not explain the reason for the cars acceleration, the distance the vehicle traveled without stopping, and the close proximity of the crash to an elementary and middle school I filed a request for Immediate Threat License Suspension/Revocation. All proper paperwork as well as a copy of this report were faxed to the Registry of Motor Vehicles.

On Wednesday 5/22/19 at approximately 1907 hours I was dispatched to the area of Oak Hill Street and Dedham Street for a report of a minor motor vehicle crash with no injuries. Upon arrival I spoke with the operator, LOZANO Jose, of MV1 (Ma Reg 8DC139) who states that he was stopped at the stop sign at the end of Oak Hill Street waiting for traffic to clear to take a left on Dedham Street when he was struck in the rear by motor vehicle #2. Operator, ROBISON James, of MV2 (Ma Reg 625XT2) states that he was traveling down Oak Hill Street toward Dedham Street coming down the hill. As he started stepping on his brakes he stated that the ABS began pushing back and he was unable to stop causing him to crash into the rear of motor vehicle 1. There were no injuries to any of the parties and motor vehicle 1 sustaind damage to the rear bumper. Motor vehicle 2 only sustained damage to its front license plate.

On 07/16/19 at approximately 09:38 I responded to a two vehicle crash on Needham St. outside of #141. The Operator of Veh #1 stated she was turning left on Needham St when the South bound traffic stopped to left her turn. As she turned, Veh#2 drove South down the multi-turn lane and they collided nearly head on. Heavy damage and air bag deployment to Veh #1. The operator was checked out by Medics and signed a patient refusal. The Operator of Veh #2 stated she was driving down the multi-turn lane South on Needham St.. She was preparing to take a left onto Jaconnet St when Veh #1 pulled out of parking lot and collided with her vehicle. I observed moderate to heavy damage to Veh #2 and noted that the vehicle didn't have its directional on. The operator of Veh #2 complained to left leg pain and was transported to Newton-Wellesley Hospital By Ambulance #1. Both vehicles were towed by Tody's Towing. At the conclusion of my investigation it appears as if both vehicles were at fault during this crash; Veh#1 for not yielding to right of way and Veh #2 for traveling too soon in a turn lane, no directional and driving too fast for the heavy traffic conditions. Both operators were given verbal warnings and there will be no citations issued at this time.

On 08/02/19 I responded to single vehicle crash across the street from #33 Dedham St. The operator of veh #1 stated he was driving due North on Dedham St. when his car veered into the city tree. There was airbag deployment and heavy damage to veh #1. Operator of veh #1 had some cuts and scrapes to legs but was checked out by the Medics and signed a patient refusal. The vehicle was towed by Tody's towing and the operator was issued Mass Citation #T1445105 in hand for CH89/ Sec4a Marked Lanes violation (traveling outside of breakdown lane) and Ch 90/ Sec 18 Speeding (based upon the significant amount of damage to the vehicle and how far the debris was found from the crash site) . The operator was picked up on scene by his daughter. Traffic Officer McSweeney arrived and took pictures of the damage to the city tree.

Operator 1 of vehicle 1 was traveling w/b on Roosevelt Rd when Vehicle 2 hit the curb and collided with her vehicle. Major damage to vehicle 1 and was towed by todys towing. On 12/1/19 I was working N491 when I responded to Washington St. at Thornton St. for a repot of an MVA. Upon arrival I observed MA Reg TCY356 stuck on top of a guardrail in front of 371 Washington Street. I spoke with the operator identified as Quinn Fay who stated that while traveling Westbound on Washington St., he lost control of the vehicle in the snow and crashed over the curb and onto a guardrail. Fay and his passenger Karlis Brava stated they were both not injured and took and uber home. Todys responded and towed the vehicle.

On 12/30/19 I responded to a single vehicle crash on Dedham St. outside of #268. The operator stated she was traveling due North on Dedham St at 25mph when she lost control of her vehicle due to icy conditions. Tracing back the vehicles tires marks in the slush, the car appeared to cross the double yellow lines near Brush Hill Rd and then back to the correct lane sideways before striking the curb, then a city tree in front of #268 Dedham St. There were multiple air bag deployments in the vehicle. Ambulance #1 arrived on scene and transported the operator due to a complaint of right chest pain from the seatbelt. N523 Traffic Officer Wilson arrived on scene and took pictures of the damage to the city tree. The vehicle was towed by Tody's Towing and the operator was mailed Mass. citation # T2013422 for Ch90/sec17-Speeding (too fast for conditions).

Operator of MV1 was traveling Westbound on Rt. 9 at a high rate of speed and veered off the road colliding with a street sign then landing in the backyard of 4 Meredith Ave damaging a fence. It should be noted MV1 failed to stop for police previously in a motor vehicle pursuit. MV1 sustained heavy front end damage. Operator sustained minor injuries and was transported to Newton Wellesley Hospital on a section 12. The vehicle was then towed by Tody's. A motor vehicle inventory form was filled out in regards. A message was left for the city to repair the street sign. Pictures were taken and placed into property and evidence. It should be noted a fence had to be removed at 4 Meredith Ave in order to removed the vehicle. I was able to get permission by the owner of the home to remove the fence.

M/V#1 was traveling east on the Rte 9 off ramp, and came to a stop, at the intersection of Chestnut St. He stated that north bound traffic, on Chestnut St. had stopped to allow him to enter the intersection, and as he did, he was struck by M/V#2. The operator of M/V#2 stated that she was traveling north on Chestnut St., and as she came over the hill, she attempted to stop, but her car began to slide. She stated that to avoid hitting the stopped traffic in front of her, she pulled into the left lane, and her M/V continued to slide downhill, past the stop sign, and into the intersection, where she collided with M/V#1. On 6/27/20 at 1742 I responded with multiple units, NFD and medics for a report of a MVA with injuries on Nahanton St. Upon arrival I observed NH Reg 4539251 wrapped around Verizon utility pole 6/15 facing eastbound on Nahanton St. The operator and the passenger behind the operator were trapped inside the vehicle. The front right passenger was able to get out of the car. Multiple witnesses stated that the vehicle was traveling WB and when it came around the bend in the road the operator lost control and spun clockwise into the pole. The vehicle ultimately rested facing the direction it came from, eastbound. NFD responded and was able to free the remaining two passengers from the vehicle. All passengers were transported to Beth Israel Boston for treatment. While on scene, I spoke with the passenger from the front seat who stated they were traveling westbound towards Needham and the operator just lost control around the bend due to the wet surface. There were no other vehicles involved in the accident, Tody's responded to tow the vehicle. Verizon was notified of their utility pole being damaged. Pictures were taken of the scene and turned over to IT to be uploaded to this report. I responded to the hospital to follow up with the occupants but was only allowed to speak with the passengers and not the operator as he was in critical condition with a broken femur and head injuries. The rear passenger stated that he didn't think his son, the operator, was going that fast but he lost control around the bend, he was suffering from neck pain and is in fair condition. The passenger in the front seat suffered minor/unknown leg injuries but hospital staff believed he would be discharged tonight.

On 9/27/20 I was Working N497 when I responded to an MVA with rollover in the area of Chestnut and Beacon St. along with multiple other units. Upon arrival at 565 Chestnut St. (which is a publicway in the city of Newton) I spoke with the operator of Veh 1 Jenna Wolf. Wolf stated that she was traveling northbound on Chestnut St. when a Vehicle traveling southbound swerved at her causing her to swerve to the left causing her to jump the curb and collide with a parked car. Wolf stated the car then rolled over and she was able to climb out on her own. Wolf had a laceration to the head that was bleeding so Newton Medics transported her to NWH for treatment. Wolf had a moderate smell of alcoholic beverages emanating from her person and glassy eyes and was slurring her words. Wolf was not asked to do field sobriety due to her injuries. The first parked car (Veh 2) that was struck by Veh 1 slid across the driveway and struck Veh 3, also causing damages to the driveway and yard. The Vehicles owner Vladimir Berezovsky was notified and provided a report number. Veh 2, and Veh 3 were disabled and towed away by Todys at the owners request. A witness named Reza Dana spoke with Ofc. T Keefe and stated "We were traveling down Beacon St. and had a green light at the intersection of Chestnut St. when two cars came speeding northbound on Chestnut St. and ran the red light at Beacon St. almost killing him". Then one of the cars went off the road and struck a parked car causing it to rollover. Veh 1 came back through dispatch as unregistered. Due to the fact that it was a residental area, the vehicle almost struck a home and the witness statment, Wolf was issued a Ma Uniform Citation T2080019 for 90/24/j Oui, 90/24/E MV Negligent Operation, 90/9/b Unregistered, 90/17/A Speeding. Pictures were taken by Ofc Keefe and submitted to IT.

On 12/05/2020 at approximately 22:00 hours I was called in to respond to a single vehicle crash with serious injuries in the area of 291 Nahanton St. Nahanton St is a pubic way in the City of Newton. Upon my arrival at the crash scene with Sgt Devine I was briefed by Officers Daly and Anderson, ( see Ofc Daly's report for further information CAD # 20054728 in regard to changes take out against the operator of the truck ) I made the following observations of the crash scene, Nahanton St is a public way in the City of Newton that is a two way roadway divided by an unprotected merdium ( double yellow line ) that runs east to west. The roadway surface was wet due snow and ice. The roadway was plowed and the snow was pushed to the shoulder, althought wet the roadway was icy. There was approximately 2 inches of snow on the shoulders of the roadway. The road was light with overhead head street lights. The posted speed limit is 25 MPH. I observed a 2013 Chevrolet Silverado pick up truck that had major front end damage. The Chevrolet was at an angle with it's front end facing west bound and resting on the grass shoulder and into a wooded area. The Chevrolet rear was sticking out into the shoulder of the east bound lane of Nahanton St. The passenger side front and rear doors of the Chevrolet had been removed by the fire department to extricate the driver and passenger. The driver and passenger side air bags were deployed . The front passenger side windshield of the Chevrolet was completely missing and the driver's side had completely shattered. The front tires, hood, and engine had been compacted on it's self. The front and rear driver's side doors had been mechanically open by the fire department. The speedometer needle had stopped at 54 MPH. The Chevrolet left the roadway and left tire marks in the snow in the eastbound bike lane of Nahanton St. At a 106.4 feet from the beginning of the tire marks the first point of impact was with two tree stumps that were 3.5 feet from the roadway, four feet tall and 3.5 feet in diameter. The tire marks continue in the east bound bike lane and exit the roadway and onto a dirt shoulder at 136.1 feet from the beginning of the tire marks. The truck continued to the right of a utility pole and then it's passenger side made contact with a tree at 170.8 feet from the begin of the tire marks. The tree was 12 feet from the bike lane approximately 5 feet in diameter. The impact with the tree caused the truck to turn so that the rear was facing north east on the dirt shoulder and bike lane. The final point of rest of the rear passenger side tire was 200 feet from the beginning of the tire marks. The truck's rear diver side tire final point of rest was 209 feet from the beginning of the tire marks. The front driver's side tire was compacted into the trucks engine and the passenger side front tire was located behind the tree approximately 20 feet from the roadway. Sgt Devine took photos of the crash area and of the truck and the disk was download by the NPD's IT Bureau. The truck was towed from the crash scene by Todys's Towing to their lot for storage, I fill out a towed motor vehicle form. There were no reported witness who observed the crash. The crash was reported by an unknown passerby. On 12/07/2020 Ofc Gaudet faxed a request for preservation of medical records, blood and urine samples of Schneider to Beth Israel Deconess Medical Center.

On Saturday January 2, 2021 at approximately 0115 hours I was dispatched to 229 Auburn St for a motor vehicle accident into a tree. MV1 was operating eastbound on Auburn St when it crossed over the marked center lane. MV1 struck the westbound side curb and jumped onto the sidewalk. On Thursday 7/15/21 at approximately 0900hrs I was dispatched to 12 Rockland Street for the report of a motor vehicle with heavy damage resting in the middle of the street. Upon arrival I observed a brown Nissan Altima unoccupied not running in the middle of street. Heavy front end damage on the passenger side along with two flat tires on the passenger side. There was no operator on scene but the car was registered to 12 Rockland Street. It should be noted that Rockland Street is a public way located in the City of Newton. I spoke to the reporting party Barry Cocuzzo who lives at 18 Rockland Street. Mr. Cocuzzo stated he witnessed William Power operating the Nissan and turn onto Rockland Street. Mr. Cocuzzo observed Power drive into his driveway with the two flat tires and heavy front end damage. When Power exited the vehicle Cocuzzo yelled over to him but stated Power's was unable to communicate clearly to him. Cocuzzo observed Power's walk into his residence at 12 Rockland and then bereaved the vehicle slowly roll back out of the driveway. Medic 3 and Tody's responded to the scene. Due to the heavy damage and two flat tires Tody towed the vehicle. A towed motor vehicle inventory report was filled out. Medic 3 was called to the scene to evaluate the health of Power's. Officer Tocci and myself located Power's in the residence passed out on the couch. Empty alcohol cans were all around Power's. After several attempts to wake up Power's he finally came too. Power's speech was severely slurred, he could barely keep his eyes upon and he lacked the ability to stand up without assistance. The Medics transported Power's to Newton Wellesley. Power's is being cited (Ma Citation 069847AB and 06850AB) for 90/10/A Unlicensed Operation, 90/24/E Negligent Operation of MV, 90/24/C Leave Scene of Property Damage, 90/24/J OUI Liquor and 90/24/P Use MV Without Authority. 5 photos were taken of the motor vehicle. It should be noted that the original location of the crash and the object that was struck are unknown to us at this time. Dispatch was also able to confirm that Power's did not have an active license at this time. Power's mother Bernice Fougere who is the owner of the vehicle stated to me that Power's did not have permission to use her vehicle.

On September 9, 2021 at approximately 10:56 hours I,Officer Guarino, responded to 79 Florence St for a report of a single vehicle crash into a gate and pole. Upon arrival, I observed MV1 with heavy front end damage crashed against a brick structure that holds the iron gate of the property. The gate was detached from the structure and on the sidewalk. The operator of MV1 told me that she was driving northbound towards Florence St when she observed a UPS truck entering the property. Operator of MV1 thought the UPS truck entered her lane of travel and swerved to the right which caused her to crash into the gate and brick structure. The operator of the UPS truck was on scene and had no damage on his vehicle. Multiple witnesses observed the crash who said that the UPS truck was traveling slow and stayed within its lane of travel. A witness said that the operator of MV1 was "flying" through the parking lot and swerved into the gate and structure. I observed the video footage at 79 Florence St where you can see MV1 traveling through the parking lot and it appeared that the UPS truck had already entered the property when MV1 took the corner and MV1 drove into the gate and Brick structure. I emailed the property manager to have the footage emailed to me, so I can attach it to this report. Witnesses, Rich (781)351-0172 and Mario (615) 981-4890 left their information with the UPS driver but I was unable to make contact. The witness I spoke with on scene wanted to remain anonymous.

Be advised at this time the weather was very rainy and cloudy, and the roads were covered in leafs and were wet. MV1 was operating on Cherry St northbound by the Derby St intersection behind MV2. MV1 operator stated he stopped as the intersection traffic light was turning yellow. MV1 operator stated he stopped with at least one car length between his MV at the intersection. MV1 operator stated MV2 continued halfway through the intersection but stopped as the light was turning yellow. MV1 operator stated after MV2 stopped in the middle of the intersection, it reversed backwards and struck his MV. MV1 sustained extensive front end damage and was towed by Todys. MV1 operator reported no injuries at this time. MV2 operator stated she was on Cherry St northbound at the Derby St intersection. MV2 operator stated when the intersection traffic lights turned yellow she stopped in the middle of the intersection, and put her MV in reverse. On Sunday 2/13/22 at approximately 1746 hours while assigned to marked unit n501 patrolling division n495, I was dispatched to the intersection of Commonwealth Ave and Homer St. for a report of a single car MVA into a traffic signal. Upon arrival I observed Ma Reg 9LL519 on the island facing eastbound in the intersection of Commonwealth Ave and Homer St. The vehicle hit and knocked the traffic signal off its base and landed in the eastbound lane of Homer Street. Operator, O'CONNOR James, states he was travelling Eastbound on Commonwealth Ave and attempted to turn right onto Homer Street. He was unable to pull out of the turn due to the snowy and icy conditions and slid into and knocked over the traffic signal. I was able to move the signal out of the lane of traffic and onto the center island and Dagle Electric was notified. Two pictures were taken and attached to this report. O'CONNOR's vehicle was inoperable and was towed by Tody's. At the time of the accident it was difficult to determine what the damage was. Most serious damage appears to be to the under carriage, rendering the vehicle inoperable.

On Friday April 8, 2022 at approximately 1230 hours I was dispatched to 36 Glen Ave for a single car MVA into a stone wall. The sky was clear at this time of day but the roads were wet due to a previous rain storm. Witness stated MV1 was operating southbound on Glen Ave (public way) prior to 36 Glen Ave at approximately 40mph. Be advised the speed limit of Glen Ave is 25mph. Witness said MV1 crossed into the northbound lane, and operated through the driveway entrance and into the stone wall belonging to Glen Ave Association. Witness stated there were no other MVs on the road at this time and they were unsure if MV1 operator was on their cellphone while driving. Witness stated MV1 operator crawled out from the vehicle and there were no one else involved or in the car. MV1 sustained heavy and extensive front end damage, and was towed by Todys. MV1 operator stated they only had chest pains, and was not suffering from any head or neck injury at this time. Due to the nature of the incident and quick response by Newton Medics, I was unable to speak to MV1 operator, who was transported to Newton-Wellesley Hospital. I was able to speak to an employee of Glen Ave Association and advised them of the incident. The employee was the manager and they stated the damage to their stone wall was approximately $5,000 USD, and their telephone extension was x200. The employee was given the accident report number.

Narrative:
On 06/15/22 at 14:47 hours, I (Traffic Unit # 523) responded to Cragmore Road (public way) and Boylston Street for a hit & run motor vehicle crash involving a yellow school bus and a tree company truck. The weather at the time was cloudy and sunny and the road conditions were dry. It appears the crash occurred at approximately 14:30 hours on this date according to statements. On arrival, I spoke to an employee identified as Alan Hayden of the tree company. Mr. Hayden works for Tree Technology and Landscape Company Inc. and at the time of the crash he was up in the air inside the bucket of his truck conducting tree trimming work. No other workers were inside the truck at the time. The truck is a 2022 Freightliner M2 bearing MA commercial reg. # W23962. Hayden states the truck was parked at the end of Cragmore Road a bit out into the Boylston Street off ramp. While doing work, Hayden states a full size yellow school bus attempted to go around his truck while turning right on to the Boylston Street off ramp from Cragmore Road. The school bus did not make a wide enough right turn and struck the driver's side black metal cage which supports the bucket to the truck. The bucket then started shaking back and forth while Mr. Hayden was in it. The metal cage goes overhead in front of the truck and sticks out on both sides of it. He says that the school bus scraped along the side of the cage and there should be long sized scrape marks along the top right side of the bus. I observed the scrape marks to the cage and noticed that the cage was also bent outward slightly on the passenger side. No injuries were reported by Mr. Hayden. I took several photos of the Freightliner truck and cage area and submitted them to the I.T. Bureau. Two other employees for Tree Technology were outside of the truck observing the work being done at the time. They were identified as Edgar Ortiz and Kemwrick Thames. Both of them did not observe the collision but heard the school bus scrape along side their company's truck. They stated that they yelled out to the bus operator but the driver continued westbound on to Boylston Street without stopping. Neither of them saw any writing on the bus or a registration number for it. They did observe some type of paper sign attached to the side of it. They stated that numerous students were on the school bus at the time. They did not observe the sex of the bus operator. Dispatch attempted to contact the City of Newton Transportation Dept. with negative results. Sgt. Wade left a voicemail with the director Regina Moody to see if the bus involved was transporting students for the City of Newton/Public Schools. It is unknown at this time if this bus was contracted by the city. Traffic Bureau update (Officer Gaudet): On Thursday, June 16, 2022, I was asked to conduct a follow up investigation for this crash. I spoke with Ms. Regina Moody from the City of Newton Transportation Department. Ms. Moody identified the operator of the school bus as Mr. Urban Mills (S49560217) and stated he was operating bus #351, a 2019 Thomas School Bus (MA BUN: BU43104). Ms. Moody was able to provide me with a copy of the audio/video surveillance from the bus involved in the crash. I viewed this footage in Ms. Moody's office. The footage shows Mr. Mills operating the school bus on Cragmore Road (S) towards Boylston Street. As the bus approached Boylston Street, a tree trimming crew can be seen working ahead at the corner of Cragmore Road and Boylston Street. There is also a stop sign at this intersection that can be clearly viewed from the footage. As the school bus approaches the intersection it does not come to a stop at the stop sign posted on the right side of the roadway. This was confirmed by Ms. Moody using the GPS system that gives the speed of every bus traveling during it's route, including when it comes to a complete stop. The school bus takes a right turn and a loud "crunching/scraping" sound can be heard. The attention of the children on the bus is turned to the passenger side of the bus. Through my training and experience, the sound heard on the footage is consistent with a vehicle hitting a large object. Children on the bus can be heard yelling, "What the fuck!" and discussing what has just happened with one another. The bus completes it's turn and travels on it's way with out stopping. All of the students on the bus were dropped off at their usual destination. There have been no injuries reported by any of the parties on the bus at the time of the crash. All of the students on the bus at the time of the crash were from Brown Middle School. Ms. Moody stated she would contact the Brown Middle School to try and get a list of the students that were on the bus at the time of the crash. A copy of the footage from the school bus and pictures of the damage to bus #351 was submitted to the IT Bureau. Mr. Mills provided a statement to Ms. Moody sometime after the crash occurred. From Mr. Mills's written statement, he states, "On approaching the intersection, I noticed a couple of tree cutting vehicles parked on the right side of the roadway (Cragmore Road) at the very corner. Nevertheless, I negotiated the turn and proceeded. This will require me to maneuver my bus onto the sidewalk. While executing this maneuver, I heard some noises which at the time I thought it was as a result of the bus getting onto the sidewalk because while executing the maneuver my eyes were fixed on the right side mirrors and was a great distance away from the vehicles to my right." After viewing the footage from the crash, Mr. Mills will be mailed Massachusetts Uniform Citation 477181AB for Chapter 90, Section 24 (Leaving the Scene of Property Damage), Chapter 89, Section 9, (Fail To Yield, Stop Sign), and Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). A copy of the criminal application and associated paperwork was submitted to the Police Prosecutor. Ms. Moody was apprised of the findings of my investigation. Cragmore Road and Boylston Street are both public ways in the City of Newton.

On Tuesday, July 19, 2022, while assigned to unit N496, I responded to the area of Boylston Street ( W) @ Dudley Road for a report of a head on collision. The weather at the time of the crash was clear and sunny. The road surface was dry. Boylston Street and Dudley Road are both public ways in the City of Newton. I observed a blue 2015 BMW 550I (NY: GUY9216) resting on the sidewalk in front of 25 Dudley Road after it crashed into the stone retaining wall in front of this address. The lone occupant of the vehicle, Ms. Tiffany Wu, was out of the vehicle when I arrived and reported no injuries. Ms. Wu stated she was operating her vehicle on Boylston Street (W) towards Dudley Road and attempted to take a right turn on to Dudley Road. Ms. Wu stated she took the turn too fast and crashed into the wall. Tody's responded and removed the vehicle from the roadway. The owner of 25 Dudley Road was present on scene and was advised about the damage to his retaining wall. Newton Medics evaluated Ms. Wu and obtained a patient refusal.
