The operator of MV1 stated that she was traveling southbound on Chestnut street when she lost control of her vehicle causing the vehicle to jump the curb and run over a city speed limit sign breaking it at the base. The city was notified of the broken sign and pictures were taken and placed in ITs mailbox to be attached to the report. The vehicle sustained a right rear flat tire and was towed by AAA. All parties reported no injuries and signed a patient refusal.

I SPOKE WITH THE OPERATOR OF MA DEALER REG 5550 MARISSA DEVLIN WHO STATED SHE HAD STOPPED FOR A PEDESTRIAN IN THE CROSSWALK WHEN SHE WAS STRUCK FROM BEHIND. I THAN SPOKE WITH THE OPERATOR OF MA PASSENGER REG 3NM859, THOMAS DORRANCE WHO STATED ' IT APPEARS I'M AT FAULT' DORRANCE STATED HE HAD NOT NOTICED DEVLIN SLOWING AND STOPPING AND RAN INTO THE BACK OF HER. DEVLIN WAS ALSO COMPLAINING OF PAIN FROM THE STEERING WHEEL AND AIRBAG, CATALDO RESPONDED. NFD AND TODYS TOW ARRIVED ON SCENE .NFD USED THE JAWS TO SEPARATE THE TWO VEHICLES TODYS WAS ABLE TO TOW BOTH VEHICLES. DORRANCE SIGNED A PATIENT REFUSAL WITH CATALDO. NFD SPREAD SPEEDY DRY AND CLEARED THE ROADWAY.

On 2/1/19 I was working N496 when I responded to a 2 car MVA at 15 Cypress St. with a road rage incident ( incident # 19004330). Upon arrival I spoke with Edward Macaleese. Macaleese stated MA reg 2EE232 (being operated by Fabian Castillo) was tailgating him while traveling southbound on Cypress St. so he pulled to the side to allow him to pass. After Castillo passed Macaleese he slammed on his brakes causing Macaleese to rear end Castillo. I spoke with Castillo who stated that he was traveling southbound on Cypress St. when the vehicle in front of him (Macaleese) began driving "much slower than the speed limit". Castillo stated the Macaleese pulled over to allow him to pass, after Castillo began to slow in traffic due to a vehicle pulling out. Macaleese struck Castillo from behind. Both parties stated they were not injured. Both vehicle were still drive able. Both parties were provided an accident report number.

Narrative:
On 3/1/19 while working N493, myself and N492 Officer Selig were sent to the area of 1354 Washington St for a motor vehicle accident with reported injuries. On arrival, I spoke to Matthew Digregorio DOB7/2/74 ( hereby referred to as operator #1) who was standing on the sidewalk next to MA reg 134TR9 (vehicle #1 ). Vehicle #1 had significant damage to the front and right quarter panels. It was straddled between the travel lanes and did not have the keys in the ignition. It was immediately apparent that operator #1 suffered an injury to his head, as there was a moderate amount of blood and small pieces of glass stuck in his forehead area. I observed operator #1 to be unsteady on his feet, have bloodshot eyes and slurred speech. I could detect a moderate odor of alchoholic beverage coming from operator #1 as I was approaching him, which was apparent from about an arms length away (it should be noted that medics were already en route to the scene at this time). Prior to the medics arrival, I asked operator #1 for a brief synopsis of what transpired. Operator #1 stated that he had just left Paddy's Pub and was driving westbound on Hunter St (which is approximately 1/8 mile from our location) when he collided head on with vehicle #2. Vehicle #2 was not immediately visible upon police arrival, however was pointed out as being located at the intersection of Hunter and Putnam St. I asked operator #1 where he was heading, as Hunter St is a back road not directly in the vicinity of Paddy's. Operator #1 could not answer the question and also remained silent when I asked him how much alcohol he had consumed this evening. At this time the medics arrived on scene. I asked operator #1 for the car keys to reliquish to the tow truck operator, which is standard operating practice. Operator #1 reached into his pocket and then provided me with the keys. Due to concern over operator #1's obvious head trauma, he was then transported to NWH to be evaluated while the investigation continued on scene. Two other parties approached myself and Officer Selig. One was the operator of vehicle #2, MA reg 1RD999. Operator #2 is identified as Luke Ferdinands DOB 5/11/80. The other party who approached is identified as Douglas Dasilva-Pimenta DOB 6/2/86 (Dasilva-Pimenta will now be referred to as operator #3). Operator #2 states that he was taking a right onto Hunter St from Putnam St, when he observed vehicle #1 on the wrong side of the road approaching him head on. Operator #2 states vehicle #1 then collided head on with him, took a right onto Putnam St, and continued onto the Washington St overpass near 1354 Washington St. Operator #3 states he was inside of #5 Hunter St when he heard a loud crash coming from outside. He looked out the window and saw vehicle #1 traveling at a high rate of speed on Hunter St towards Putnam St. Operator #3 observed damage to the right side of his parked vehicle (now referred to as vehicle #3) MA reg 7CAK80. He immediately got in his vehicle and proceeded to follow vehicle #1, which then crashed into vehicle #2 head on. Operator #1 did not stop until it was obvious that operator #3 was following close behind. This ultimately landed vehicle #1 at it's final location near 1354 Washington St. Vehicle #1 and vehicle #2 were not driveable and were towed to Tody's. It should be noted that there was a passenger in vehicle #2 at the time of the collision. Airbags were deployed and medics checked out both operator and passenger. Both declined transport. The passenger is identified as Elizabeth Ferdinands DOB 5/22/53.Ferdinands' version of events coincide with that of operator #2. Upon speaking with Officer Selig it was learned that operator #1 dropped his cell phone several times as he was holding it on scene. Both Officer Selig and operators #2 and #3 observed this. Operators #2 and #3 both expressed concern over the obvious odor of alcohol eminating from operator #1, and the fact that he was unsteady on his feet. I responded to NWH to follow up with operator #1 however he did not wish to speak further. He did express concern over the well being of all parties involved. Operator #1 was issued in hand the following citations: T1268426 - 90/24 OUI Alcohol, 90/24 Leaving the Scene (property damage, vehicle #2), and 90/24 Leaving the Scene (property damage, vehicle #3). T1268425 - 89/4a Marked Lanes violation. T1269216 - 90/24 Negligent Operation of a M/V (mailed after consultation with superiors) All streets mentioned are public ways in the City of Newton. 1354 Washington St will be the address used on this report and subsequent citations due to multiple locations being involved. Citations and paperwork have been placed in the prosecutor's mailbox. An immediate threat form has been filled out and faxed to Mass DOT. *Attention Police Prosecutor for blood work subpoena, if applicable*

MV#1 stated while traveling westbound on Commonwealth Ave he closed his eyes for a few moments causing his vehicle to veer to the right. MV#1 drove up on the curb and struck a overhanging street light. MV#1 sustained heavy front end damage as well as uprooting the overhanging street light. Operator of MV#1 stated he had minor injuries and signed a patient refusal. Operator of MV#1 was given the following roadside tests, due to the time and circumstances of the incident. Horizontal Gaze Nystagmus - Pass One Leg Stand - Pass Walk And Turn - Pass Operator of MV#1 showed no signs of impairment. Through my training and experience I calculated MV#1 had speeds of approximately +50MPH. Tody's towed the MV, a towed MV inventory form was completed. Pictures taken on scene submitted to IT.

Operator of MV#1 stated she was travelling east in the area of 1507 Washington St in the southbound lane. She began to slow down in traffic when she was rear ended by MV#2. MV#1 suffered minor damage to the back and was able to be driven from the scene. Operator of MV#2 stated she was also in the southbound lane and traffic was travelling at a normal speed when she turned to look left to switch lanes. She stated at that moment "traffic began to slow down into gridlock" and she could not stop in time. She rear ended the back of MV#1. MV#2 suffered major damage to the front end and was towed from the scene by AAA (CJ's Auto.) No injuries.

Mrs. Weiss stated that she was attempting to back out of her driveway at 285 Crafts Street when her vehicle accelerated for no reason "like a bat out of hell." The vehicle then crossed Crafts street at a high rate of speed, jumped the curb, crossed the large grass island, and only stopped when the vehicle crashed into a tree. Mrs. Weiss was uninjured and signed a patient refusal. The vehicle sustained moderate damage to the tires, undercarriage, and rear bumper and was towed from the scene by Todys. Photos were taken of the damaged city tree and were placed in the IT bureaus mailbox to be attached to this report. Based on the facts that Mrs. Weiss could not explain the reason for the cars acceleration, the distance the vehicle traveled without stopping, and the close proximity of the crash to an elementary and middle school I filed a request for Immediate Threat License Suspension/Revocation. All proper paperwork as well as a copy of this report were faxed to the Registry of Motor Vehicles.

On 05/15/2019, while assigned to N494, I, Officer Conary, responded to a MVA at 109 Adam Street, a public way in the City of Newton. Upon arrival, I observed a utility pole broken in half, wires hanging in the street, and also a transformer on the ground. Fire and medics responded. Other police units were able to block the street from traffic. I spoke to witness, HARRIS, who stated that she was traveling Southbound on Adam Street when she saw MV1 in the wrong lane traveling toward her at a high rate of speed. HARRIS stated that MV1 then turned into the utility pole located outside of 109. MV1 took out the pole, side swiped MV2 and then hit MV3 head on. Operator of MV1 stated that he was traveling Northbound on Adam Street when he sneezed. At this point, he said his eyes were off the road and the next thing he could remember was being in the parking lot of 109 Adam Street after the accident. There was significant damage to the front of MV1 with airbag deployment. Operator of MV1 was checked out by the medics for a head injury and signed a refusal. Operator of MV1 was asked if he had consumed any alcohol or taken any medication today and he declined. Officer Spinney (N491) asked if he was willing to do a Field Sobriety Test, and he stated yes. Operator of MV1 did not show any signs of intoxication. Operator of MV2 stated that he had parked his car outside of 109 Adam Street and was sitting in the driver seat when he was hit by MV1 and also the utility pole. Operator of MV2 was able to exit his car. He was evaluated by the medics and signed a refusal. MV2 has significant damage to the right side and windshield. MV3 was parked in the lot of 109 Adam Street facing Southbound. MV1 hit MV3 head on. There was significant damage to MV3 front end. The electric meter attached to 109 Adam Street was ripped off the house. There are no signs of damage to the house to report at this time. MV1 was towed by Tody's. Eversource arrived on scene. Operator of MV1 was issued a citation for marked lanes violation. Pictures were taken of the scene and will be submitted to IT.

Operator of MV1 states he was stopped in traffic westbound, in the area of 1045 Beacon St. He states suddenly he was struck from behind by MV2. She states she tried to stop her car but it was not responding. She attempted to downshift her car and put it in park, but instead it lunged forward into MV1 multiple times. DUE TO NEWTONVILLE AVE BEING A NARROW RD WHEN CARS PARK ON THE EAST SIDE WHICH IS LEGAL THEY(PARKED CARS ) TAKE UP THE WHOLE EAST SIDE OF THE ROAD. BASED ON MS ROSENSWEIG STATEMENT I FEEL SHE HAD THE RIGHT OF WAY. VEHICLE #2 SHOULD HAVE YIELDED SEEING THAT VEHICLE #1 HAD NOWHERE TO GO. On Tuesday, July 23rd 2019, at approximately 5:24pm, I, Officer Brooks, responded to 444 Lowell avenue for a 2 car MVA. Upon my arrival I observed MV1(NH REG 4120041) with heavy front end damage. The vehicle was located in the driveway of 444 Lowell avenue, with the front end sticking out into the roadway. I spoke with the operator of MV1 who stated he was trying to exit the driveway and turn left onto Lowell avenue southbound, when he was struck by MV2(MA REG 3AP243) which had been traveling northbound on Lowell avenue. According to the operator of MV1, MV2 had been traveling at a high rate of speed and tried to swerve around him. There was airbag deployment in MV1 however the operator signed a patient refusal with Cataldo. MV1 was towed by Tody's. I then spoke with the operator of MV2 who stated she was traveling northbound on Lowell avenue and saw MV1 exiting the driveway. She assumed he was going to stop pulling out of the driveway, and when he did not she stated she attempted to swerve around him. MV1 made contact with the passenger side rear wheel well of MV2 causing MV2 to spin out. The operator was not injured and arranged for a private tow due to the damage in the wheel well area.

On 08/02/19 I responded to single vehicle crash across the street from #33 Dedham St. The operator of veh #1 stated he was driving due North on Dedham St. when his car veered into the city tree. There was airbag deployment and heavy damage to veh #1. Operator of veh #1 had some cuts and scrapes to legs but was checked out by the Medics and signed a patient refusal. The vehicle was towed by Tody's towing and the operator was issued Mass Citation #T1445105 in hand for CH89/ Sec4a Marked Lanes violation (traveling outside of breakdown lane) and Ch 90/ Sec 18 Speeding (based upon the significant amount of damage to the vehicle and how far the debris was found from the crash site) . The operator was picked up on scene by his daughter. Traffic Officer McSweeney arrived and took pictures of the damage to the city tree.

Mv#1 operator states she was pulling out forward W/B from a parallel marked parking space in front of 843 Beacon St. #1 states she was about half way into the travel lane coming out of her turn to proceed straight ahead. #1 states at this time, she was struck on her driver's side front end that took off the bumper by Mv#2 that was travelling past her at a high rate of speed from behind straight ahead on Beacon St W/B. #1 operator further states #2 had crossed over the double yellow line to drive around her as she was out into the travel lane. Mv#2 operator states she was travelling on Beacon St W/B and was struck on her passenger side by #1 as #1 was pulling out of the parking space. I observed the initial contact damage on #2 on its passenger side rear door running down to the front of the passenger side wheel well and passenger side rear tire (tire wall partly sliced from impact). When #2 operator was asked if she crossed over the double yellow line, #2 replied only after the impact. #2 added prior to the collision, she briefly looked at her GPS. After statements were received from both operators and at the conclusion of this investigation, a witness identified herself on scene. The witness stated she was sitting in her driver's seat in her parked Mv directly in front of #1. Witness stated she observed #1 pull out from behind her into the travel lane. Witness stated #1's front end had almost fully reached her driver's side door before #2 made impact. Based on statements and witness statements and it appears #2 operator did not see #1 enter the roadway until the last second before impact by overcorrecting and entering the opposite lane of travel.

MV1 jumped a curb and collided with a parked vehicle MV2 (a 2004 GMC Yukon, MA Reg: 8EC792) in front of 185 Waltham St. MV1 operator, Angel Gonzales, was unconscious on scene in the driveway of 185 Waltham St. Waltham St is a public way that is owned and maintained by the City of Newton. A passerby, Joseph Anderson saw the aftermath of the crash and called 911. MV2 had damage to its drivers side door and rear side. MV1 was heavily damaged, on its side, and keys were in the ignition. MV1 was towed by towed by Tody's. Gonzales was transported to Beth Isreal in critical condition. When Gonazales was in the back of the ambulance, myself and medics smelled an odor of an alcoholic beverage emanating from Gonzales. All first responders thoroughly checked the surrounding area for a helmet but one could not be located. Due to Gonzales' critical condition and urgency to get to the hospital, and ID was not thoroughly looked for on his person. Ofc. Guarino, N491, retrieved an ID from Gonzales upon arrival at Beth Isreal Hospital. Ofc. Stake filled out a towed inventory form. Pictures taken and sent to IT. Sgt. Lee was notified and will be following up with this incident. MV1 operator, Angel Gonzales is being charged with the following (MA Uniform Citations T1442823 & T1442824): 90/24J - OUI Liquor (public way) 90/24E - Negligent Operation of a MV (operating recklessly at a high rate of speed) 89/4A - Marked Lanes 90/23 - Attaching Registration Plates (attaching registration plates to motorcycle to conceal the fact that it was unregistered. Registration plate on the motorcycle came back to a 2002 Honda VT750 motorcycle, MA Reg: 1T3988; VIN on the motorcycle came back to a 2005 Suzuki GSXR60, MA Reg: 1L4389) 90/9B - Unregistered MV (motorcycle 2005 Suzuki MA Reg: 1T3988 was confirmed unregistered by RMV) 90/23D - Operating MV with a Suspended License (RMV confirmed that Gonzales' license was suspended S94722414) (see Incident #19039806 for more details). For QED purposes, the charge 266/139 - Stolen Registration was added to this report because 90/23 - Attaching Registration Plates was not an option. Due to Gonzales' hospitalization, the citations will be mailed to his last known address. Officers were able to rise a Harold Curtis, the father of Anthony Curtis (owner of MV2, the GMC Yukon) at 185 Waltham St. He was notified of the incident and advised to call their insurance company. Curtis mentioned that he has cameras facing the street where the incident took place and will see if he can retrieve the footage for us. Police prosecutor to subpoena blood.

On September 16, 2019 at approximately 17:50 hours I, Officer Guarino, responded to the area of 749 Beacon St. for a report of a motor vehicle crash involving a bicycle. Upon arrival with other units, we determined that MV 1 was parked in front of 749 Beacon St.The operator of MV1 opened the door to her vehicle to exit and her door struck a passing bicyclist. Traffic on Beacon St. westbound was stopped, the bicyclist fell off of the bicycle and sustained injuries and was transported to Newton-Wellesley Hospital for further evaluation.As the bicyclist was falling, he struck the rear of MV2, which did not appear to have any damage. The operator of MV1 said she did not see the bicyclist, as she went to exit her vehicle. Witness 1, who was stopped in traffic said that the bicyclist was traveling at a high speed and understood how MV1 did not see the bicycle coming. Officer D. Siegal transported the bicycle to the Newton Police Department and secured it in support services.

HE SUFFERED A MINOR RIGHT FOOT INJURY AND WAS TRANSPORTED TO ST. ELIZABETHS. BY MEDIC 1. ALL PASSENGERS OF MV1 REPORTED THAT THE OPERATOR WAS NOT SPEEDING AND THE PEDESTRIAN DARTED INTO THE STREET. THE OPERATOR HAD DASH CAM FOOTAGE OF THE ACCIDENT, AND IT SHOWED MV1 TRAVELING WESTBOUND ON BEACON ST APPROACHING THE VEHICLE PULLED TO THE SIDE OF THE ROAD. IT THEN SHOWED THE PEDESTRIAN RUN INTO THE STREET WITHOUT LOOKING, GETTING STRUCK BY THE FRONT PASSENGER SIDE OF MV1 AND FALLING TO THE GROUND. At approximately 0147 hours I was dispatched to the area of 250 Waltham St for report of an MVA. Upon my arrival I spoke with the involved parties. The operator of vehicle 1 stated that as she was traveling southbound on Waltham St she noticed a vehicle approaching at a high rate of speed from her rear. This vehicle tailgated her for a short time, until she attempted to pull over to the side of the road to let the vehicle pass. As she was signaling her desire to do so, she was struck in the rear by vehicle 1. Vehicle #1 Was Traveling Southbound on Parker St. Vehicle #1 collided with a pedestrian on it's left drive side in a marked crosswalk in front of 21 Parker St. The operator of vehicle #1 stated she saw the pedestrian through her driver's side window at the same time she heard a loud bang. The operator of vehicle #1 stated she noticed her mirror was gone and realized she had hit something so she stopped. The pedestrian in the crosswalk was crossing from his residence at 21 Parker St. on the west side of Parker to the east side of Parker St. in a marked crosswalk. The pedestrian was approximately 2 feet from the middle divider when he was hit by the mirror of vehicle #1. It should be noted that the pedestrian was wearing all dark clothing. The Pedestrian appeared to suffer an injury to his arm and his body and was transported to Beth Israel by Cataldo. The Witness stated he did not see the crash, but just prior to it he saw an unknown vehicle driving at a high rate of speed northbound on Parker St. and the vehicle may have beeped at the pedestrian in the crosswalk causing him to step back into the southbound travel lane where the crash happened. Photographs were taken of the scene and submitted to the IT bureau to be attached to this report. The operator of Vehicle #1 was cited with MGL Ch. 89-11 fail to yield to pedestrian in a crosswalk (Citation #T2079123).

On 11/12/2019 at 12:26 hours, I responded to the Star Market parking lot located at 2040 Commonwealth Avenue for a motor vehicle pedestrian accident. The parking lot is a public/thru way that motorists use to access the Star Market along with other businesses. The weather at the time was cloudy and rainy and the parking lot was saturated. Prior to my arrival, the pedestrian involved left the scene and did not want to speak to the officer nor wanted medical attention. Ofc. Claflin then cancelled Cataldo Ambulance. The pedestrian, Mary Alexander, left her information with a store employee prior to leaving. I spoke to the operator of MV # 1 (Joshua Randel MD Lic.# R534440603244) who stated that he was travelling through the third row of the Star Market parking lot at an appropriate speed. He was then attempting to make a left turn near the entrance/exit of the store. He says that there was a large white van parked in the end handicapped spot towards the left of him. He says the van was sticking out in the spot limiting his visibility. As he was half way through his left turn and coming around the corner, he was not able to see a pedestrian pushing a shopping carriage. He then struck the carriage that was being pushed by Mary Alexander. He further says that the cart jolted Ms. Alexander and she fell down to the ground. At the time, Alexander was walking through the lot near the store entrance heading back towards the parked cars. She was not in the crosswalk at the time which was approximately ten feet down from her location. After, I attempted to go to Ms. Alexander's residence to follow up. I spoke to her friend who was bringing up Alexander's groceries and she stated that she decided to go to Newton Wellesley Hospital for treatment. I then spoke to Ms. Alexander in the lobby of the hospital. She stated that she was exiting the store through the lot with her shopping carriage. She believes she was approximately eight feet away from the door when all of a sudden she only saw red coming at her. She believes MV # 1 was speeding at the time. I asked her if she was struck by the vehicle or was the shopping carriage only struck. She replied that she did not know and only remembers falling to the ground after. She sustained a large bruise to the inside palm area of her right hand and her left knee was swollen. Digital photos of the scene were taken and pictures of Ms. Alexander's injuries were taken at the hospital. As far as possible video surveillance, I was told that the Loss Prevention Department has to access the video. On 11/13/19 at approximately 14:20 hours, I went in to the Star Market and spoke to the manager, Nidal Rajeh, inquiring about possible video surveillance. He said there is coverage in the parking lot and he was able to show me the video. A copy of the video would have to be done by Loss Prevention which I requested. I observed at the 12:12pm mark in the video, MV # 1 was parked five spots up from the third row of the lot that he was exiting. I also observed a white transport van parked in the end handicapped spot mentioned by the operator but it was parked correctly and not limiting visibility. Next, MV # 1 backs out of the spot and drives forward going at a reasonable speed. The pedestrian (Alexander) has already exited the store with her carriage and is walking at an angle toward the same third row of parked cars. As the pedestrian is walking towards the direction of MV # 1, it appears that the pedestrian should be visible to him. MV # 1 then starts to take the left turn around the corner without slowing or stopping and strikes the carriage head on. Due to the impact, Alexander then loses grasp of the carriage and it goes past her twisting her body around. She then hits and rolls up on the hood of the vehicle on her left side. MV # 1 then suddenly stops and immediately gets out and provides assistance to the pedestrian. Due to the video surveillance, Mr. Randel will be issued/mailed MA Uniform Citation # T2012347 for Ch. 90/14/A Failure to slow for a pedestrian.

No reported witnesses.

On Tuesday, December 10, 2019, while assigned to Traffic unit N525 I responded to the area of Walnut Street and Lake Street, Newton for a report of a motor vehicle crash involving a City of Newton vehicle. The weather at the time of the crash was overcast. The road surface was wet. Walnut Street and Lake Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Joseph Ratta (S82871504). Ratta was operating a City of Newton owned 2017 Ford F350 (MA MVN: M99236) Southbound in the area of 1141 Walnut Street. Ratta stated while in Traffic, MV2 crashed into the rear of his vehicle. I observed minor damage to the rear bumper area of MV1. No injures were reported. Ratta is employed by the Utilities Division. I spoke with the operator of MV2, Bryan Izaguirre (S64158637). Izaguirre stated he was operating a 2016 Honda Civic (MA: 8ZP172) Southbound in the area of 1141 Walnut Street, Newton. Izaguirre stated the front bumper of his vehicle crashed into the rear of MV2 at a low speed while they were in traffic. No injuries were reported. Pictures were taken and submitted to the IT Bureau. Izaguirre was issued Massachusetts Uniform Citation T2012561 for Newton City Ordinance Violation Chapter 19, Section 75 (Failure to Use Care).

The operator of MV1, Nicole Laurin, sates that she was driving southbound on Sheridan Street when she lost control of her vehicle causing her to crash into MV2 which was parked in front of 107 Sheridan Street. Both vehicles sustained major damage and airbag deployment and were towed from the scene by Tody's. Due to the high rate of speed of the crash and air bag deployment, the operator of MV1 was transported to Newton Wellesley Hospital to be evaluated. It should be noted that prior to the reported crash there was a 911 call regarding a white sedan traveling at a high rate of speed going through numerous traffic signals and signs in a densely populated neighborhood with narrow streets. After the crash several more 911 calls were received stating that a vehicle traveling at an excessive rate of speed just crashed into a parked vehicle. Based on these calls and the nature of the crash, Ms. Nicole Laurin was issued in hand Massachusetts Unifrom Citation T2013231 for 90-24, Operating to Endanger, and 90-17, Speeding over the reasonable and proper rate. Sheridan Street is a public way in the city of Newton.

RICHARDSON admitted he was attempting to back up and align with the gas pump when he accidently struck LUONIS car. RICHARDSON additionally admitted to having an expired license and registration out of NH and no insurance, RICHARDSON also had a valid rental agreement for the UHAUL trailer TX reg AO37011 with insurance . RICHARDSON was able to re register his van on scene and provided me with an emailed copy of confirmation of registration renewal and his passenger Brendan SEIGAL who had a valid NH license was able to drive . RICHARDSON issued a criminal application on Mass Uniform Citation T2015308 for violation of MGL 90/10 Unlicensed Operation and 90/9 Operating Unregistered MV .

The operator of vehicle 1 stated she was traveling West on the Washington Street bridge over the Mass Pike when vehicle 2, a gray SUV attempted to change lanes, as this happened the front passenger side quarter panel of vehicle 2 came in contact with the rear drivers side quarter panel of vehicle 1. This caused minor scratches to vehicle 1. The operator of vehicle 1 attempted to pull over and exchange information with the other driver. The operator of vehicle 2 continued driving away and got onto the Mass Pike, during this process the operator of vehicle 1 and the passenger of vehicle 1 were able to get the license plate number of vehicle 2 which was NY REG:T797800C. In an attempt to locate and identify the driver of vehicle 2 I ran NY REG T797800C which comes back registered to a short term rental company called Flexdrive. I called a phone number on their website 8663539399 which is listed as the number to call if you are in an accident with one of their vehicles. I was only able to speak to a service technician, a Tom Louis of ARI Fleet Service 18002272273, who told me the vehicle was out of the Flexdrive North Dighton Mass hub located at 123 Williams Street North Dighton MA 02764.Tom also mentioned the location would most likely not be open again until Monday. He did not have any other information regarding who may be operating the vehicle. I have attempted to find contact information on this North Dighton location which I have had negative results so far. I have also emailed the Flexdrive customer service department. To be followed up on. Traffic Bureau update (Officer Gaudet): On Tuesday, February 25, 2020, I made contact with the registered owner of MV2, Flexdrive. The representative from Flexdrive stated MV2 is registered to the company but Lyft uses their vehicles for ride sharing. I contacted Lyft and spoke with a party named Jonas from their support team. Jonas stated he would make contact with the operator of MV2 and ask them to contact me to give a statement. Lyft stated they would also conduct their own internal investigation. On Wednesday, February 26, 2020, I spoke with the operator of MV2, Gary Procaccini (S36066201). Procaccini stated he was operating a 2019 Santa Fe (NY Reg: T797800C) as a Lyft driver on the evening of February 22, 2020. Procaccini stated he was traveling on Washington Street (W) towards the Massachusetts Turnpike entrance in West Newton. Procaccini stated he was in the wrong lane and looked in his mirror before attempting to merge into the correct lane. Procaccini stated to the right of his vehicle he observed a vehicle "flying" towards the entrance to the Pike. Procaccini stated he came to stop to allow the vehicle to pass. Procaccini stated the vehicle passed at a high rate of speed and he continued on his way onto the Pike (E). Procaccini stated he had a passenger in the vehicle using Lyft servies and there were no signs that a crash had taken place. Procaccini's information has been added to his report to reflect him being the operator of MV2. I then spoke with Peter Houser who is a Driver Services Representative with Lyft. On Tuesday, March 31st 2020, at approximately 5:11pm, I, Officer Brooks, responded to 580 Washington street for a motor vehicle accident. Upon arrival I spoke with the operator of MV1(MA REG 5TRL60) who stated he was pulling out of the parking lot of 580 Washington street, and was attempting to turn left to head westbound on Washington street. He stated he was already in the first eastbound lane of traffic when MV2( MA REG 1KL458) came at a high rate of speed eastbound on Washington street in the center lane and struck his vehicle. After striking his vehicle, MV2 hit MV3(MA REG7BG273) which was parked legally in front of 580 Washington street. The collision caused MV3 to hit MV4(MA REG 3AGN11) which was parked legally in front of MV3. MV1 had damage to the front passenger side bumper. I then spoke with the operator of MV2 who stated she was traveling eastbound on Washington street in the far right lane. She states MV1 pulled out of the parking lot of 580 Washington street and struck her vehicle causing her to loose control and hit MV3 which was parked. Her car had airbag deployment and damage to the passenger side rear wheel well as well as the front end. MV 3 had damage to the rear end and front end. MV4 had minor scratches to the rear bumper. Nobody was injured in the accident, and the operator of MV2 signed a refusal with Cataldo. Tody's towed MV2 due to the damage. A motor vehicle inventory was filled out and filed.

The OP. of MV#1 was unable to give me a statement due to a possible head injury. He kept repeating "where's my car" and "what happened?" Witness#1 (Mr. Regan) states he was travelling westbound on Auburn St. when MV#1 went to the oncoming traffic lane and passed him on the left at a high rate of speed. He estimates the speed at 70mph. He saw MV#1 hit the speed bump on Auburn St., possibly losing control, and then onto Evergreen Ave. with a large cloud of dust. Witness#2(Mr. Weishaus) was on his balcony and observed MV#1 travel at a high speed of rate on Auburn St. He estimates the speed at 100mph. He observed MV#1 go up Evergreen Ave., launch into the air and crashed. Witness#3(Mr. Litvak) states he was on his porch and heard a loud sound of a motor engine approaching. MV#1 then crashed into his retainer wall and flew through some trees before landing on his neighbor's yard. Based on witness statements and the accident scene, it appears that MV#1 went off the roadway and struck a retainer wall at #15 Evergreen Ave, launched into the air and struck parked MV#2 in the process, went through some trees that flipped the car around before landing on the property of #23 Evergreen Ave. The OP. of MV#1 was ejected from the vehicle at some point in time. Pictures were taken of the accident scene.

On May 15, 2020 at approximately 22:26 hours Officer Stake and I responded to 163 Suffolk Rd for a hit and run that had just occurred. Upon arrival, we observed MV1,a black Cadillac Escalate with heavy rear end damage parked facing westbound in front of 163 Suffolk Rd. MV1 also was had damage to the passenger side due to it being pushed into a utility pole. MV1 was parked and unoccupied at the time of the crash. I spoke with the residents of 170 Suffolk Rd, who said that they heard the crash and went outside and saw a dark colored Jeep Wrangler leaving the scene at a high rate of speed. They described the Jeep Wrangler as a "newer" model, possibly a four door with a hard top. The residents of 163 Suffolk Rd have a camera system and will view the footage at a later time. Units checked the area for the Jeep Wrangler but were unable to locate it. Todys's responded and removed MV1 from the scene. **Update by Ofc. Hamann** At 0600 hours, Dispatch advised me that a male party that was involved in an accident last night (5/15/20). Brett Catlin stated that he was driving home from CVS last night and admitted to looking down at his phone and then had crashed. He stated after he crashed he drove to his home, which is 4 houses down. He stated he had called the owner of the vehicle and apologized and stated he was going to report it in the morning. On Wednesday, 7/1/2020, at 2230hrs, I was dispatched to the area of 856 Watertown St for a report of a possible car crash. Watertown St is a public way as it is owned and maintained by the City of Newton. Upon arrival, I met with operator, Henok Kiflit, who was out of the vehicle and very disorientated. I observed a gash on his head right above his eye and noticed all airbags had been deployed in his vehicle. I asked him to sit down and I radioed for an ambulance. Kiflit stated that he was driving EAST on Watertown St when he collided with a unoccupied parked car (a red 2015 Jeep Wrangler, MA Con R22519) which was parked on the street outside of 856 Watetown St. Kiflit was driving his girlfriend's car (a gray 2020 Nissan Sentra, MA Reg 1ANN57) and was on his way home in Watertown. When I asked for Kiflit's license, he told me that he only has a permit and has yet to take the test. He apologized repeatedly. Kiflit did not have any passengers in his vehicle. Kiflit said the roadway was so dark and he didn't see the parked Jeep. He claimed he was not speeding. There was minimal damage done to the drivers side rear bumper of the Jeep. The Nissan had extensive front end damage and was subsequently towed by Tody's Towing. Towed motor vehicle form filled out and filed, copy made for police prosecutor. Owner of the Jeep came out and took possession of the vehicle. Kiflit was taken to NWH for evaluation of his injuries. Kiflit's driver history shows that he had an surchargable accident recently 05/30/2020. Massachusetts Uniform Citation T2080927 was filled out and prepared to be mailed to Kiflit at his Watertown address. Henok Kiflit is being cited for: 90/10 - Operating without a License (Learner's Permit violation; Kiflit, operator, was not accompanied by any motorist)

City of Newton DPW employee Matthew Dagostino stated that on 08/06/2020 at 10:13 hours he was driving a 2007 International 7400 dump truck south bound on Walnut St in the area of number 457. Walnut St is a public way in the city of Newton. Dagostino stated that the truck was equipped with a sander and it's gross weight was appropriately 26,000 lbs. Dagostino stated that the Subaru stopped to let a pedestrian cross the street. Dagostino stated that when the Subaru stopped abruptly it caught him off guard because there was no marked crosswalk in the area. Dagostino stated when the Subaru stopped short he applied the breaks to his truck and had to turn to the left in an attempt to avoid a crash. Dagostino stated that his truck's front passenger side bumper made contact with the Subaru's rear driver's side bumper and tail light. Dagostino's truck had small scrapes to the front passenger side bumper. Nether operator stated that they were injured and the vehicles were not towed.Lev's vehicle had damage to the rear driver's side bumper and tail light . I took photo's of both vehicle and this disk was tuned over to the NPD's It bureau for downloading. Operator of vehicle two Naomi Lev stated that she was driving south bound on Walnut St when she said she was attempting to turn left into Berwick Rd and stopped to let a pedestrian across over Walnut St . It should be noted that Berwick Rd is approximately 1/2 of a mile south of where the crash occurred. Lev stated that she did stop her vehicle to let a pedestrian across just prior to the crash.

On Tuesday, September 1, 2020, while assigned to marked unit N496, I responded to the area of 780 Beacon Street, Newton for a report of a motor vehicle crash involving a bicyclist. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of the vehicle involved in the crash, Dina Hudson (S33833807). Ms. Hudson stated she was sitting in her 2015 white Jeep Grand Cherokee (MA: 727SZ7) that was parked (E) in a metered parking spot in front of 780 Beacon Street. Ms. Hudson stated traffic in the Eastbound lane of Beacon Street to her left was stopped. There was a red light ahead at the intersection and traffic had built up past where her vehicle was parked. Ms. Hudson stated she opened her driver side door while she was looking for her debit card to go to the bank. Ms. Hudson stated at this time a bicyclist was traveling at a high rate of speed around the stopped traffic towards the intersection and crashed into her driver side door. Ms. Hudson stated the operator of the bicycle then traveled further up the roadway and crashed behind a vehicle stopped in traffic. Ms. Hudson stated she was unsure if the bicyclist crashed into the stopped vehicle or if it had landed on the roadway just before it. Ms. Hudson stated the bicyclist then got up from the roadway and began yelling at her that the crash was her fault. I observed a few small fresh scrapes on the outside of the driver side door area. When I initially arrived at the crash location, I observed Officer Lazarakis checking on his well-being of the injured bicyclist, Marc Lind. Lind was observed laying on the sidewalk in front of 780 Beacon Street. After being evaluated by Newton Medics, I spoke with Mr. Lind. Mr. Lind stated he was traveling on his Trek bicycle Eastbound on Beacon Street towards Langley Road. Mr. Lind stated while traveling through the area at this time, traffic in the Eastbound Lane was stopped and there was a red light ahead of him at the intersection. Mr. Lind estimated his speed at this time to be 10-12 miles per hour. Mr. Lind stated as he travelled past the Santander Bank at 780 Beacon Street, the operator of a parked white Jeep to his right abruptly opened their door into traffic. Mr. Lind stated he attempted to avoid the vehicles door and veered to the left. Mr. Lind stated the right end of his handlebar made contact with the door and as he traveled forward and applied his brakes his right side crashed into the rear of a vehicle stopped in traffic ahead of him. Mr. Lind stated he crashed to the roadway on his right side. Mr. Lind was able to move to the side of the roadway and wait for Newton PD, Fire, and Medics to respond. Mr. Lind reported injuries to his right arm, side, and leg area. Mr. Lind signed a patient refusal with Newton Medics and was transported away from the area by his wife with his bicycle. I observed no substantial damage to his Trek bicycle. At the time of the crash, Mr. Lind was wearing a white t-shirt, black shorts, black, shoes, and a gray helmet with sunglasses. I spoke with a witness to the crash, Steve Worthington. Mr. Worthington stated he was stopped in traffic on Beacon Street about 4 vehicles back from the red light. Mr. Worthington stated he looked out of his passenger door mirror and observed a bicyclist "whipping" down Beacon Street in between vehicles parked on the side of the road and vehicles stopped in traffic at the red light. Mr. Worthington estimated the speed of the bicycle to be approximately 20 miles per hour. Mr. Worthington stated the bicyclist had to "lean" by his vehicle to get by. Mr. Worthington stated a vehicle just ahead of him that was parked opened their door and the bicyclist made contact with the door. Mr. Worthington stated the bicyclist then went down in front of the car that was in front of him. Mr. Worthington stated the bicyclist was rolling around on the ground and screaming "It's your fault" at the woman that opened her door Photos were taken of the bicycle and the vehicle involved and submitted to the IT Bureau.

She was transported by Newton Paramedics to Newton Wellesley Hospital. The operator of MV#2 had a small dog with her that was located in the front passenger seat. Newton Animal Control took possession of the dog and transport it to the veterinarian. It was reported that the dog sustained possible internal trauma. The operator of MV#3 stated he was pulling out of the driveway of Walgrens onto Walnut St and struck MV#2. The operator of MV#3 stated MV#2 was travelling at a high rate of speed. MV#3 sustained heavy front end damages. There were no reported injuries to the operator of MV#3. Tody's Towing took possession of MV#2 and MV#3. MBTA Police Officer Michael Bolotin arrived on scene and documented the accident (MBTA case #2020-5909). MBTA Officers took possession of MV#1. The City of Newton DPW's street sweeper cleaned the debris on the street.

On 9/27/20 I was Working N497 when I responded to an MVA with rollover in the area of Chestnut and Beacon St. along with multiple other units. Upon arrival at 565 Chestnut St. (which is a publicway in the city of Newton) I spoke with the operator of Veh 1 Jenna Wolf. Wolf stated that she was traveling northbound on Chestnut St. when a Vehicle traveling southbound swerved at her causing her to swerve to the left causing her to jump the curb and collide with a parked car. Wolf stated the car then rolled over and she was able to climb out on her own. Wolf had a laceration to the head that was bleeding so Newton Medics transported her to NWH for treatment. Wolf had a moderate smell of alcoholic beverages emanating from her person and glassy eyes and was slurring her words. Wolf was not asked to do field sobriety due to her injuries. The first parked car (Veh 2) that was struck by Veh 1 slid across the driveway and struck Veh 3, also causing damages to the driveway and yard. The Vehicles owner Vladimir Berezovsky was notified and provided a report number. Veh 2, and Veh 3 were disabled and towed away by Todys at the owners request. A witness named Reza Dana spoke with Ofc. T Keefe and stated "We were traveling down Beacon St. and had a green light at the intersection of Chestnut St. when two cars came speeding northbound on Chestnut St. and ran the red light at Beacon St. almost killing him". Then one of the cars went off the road and struck a parked car causing it to rollover. Veh 1 came back through dispatch as unregistered. Due to the fact that it was a residental area, the vehicle almost struck a home and the witness statment, Wolf was issued a Ma Uniform Citation T2080019 for 90/24/j Oui, 90/24/E MV Negligent Operation, 90/9/b Unregistered, 90/17/A Speeding. Pictures were taken by Ofc Keefe and submitted to IT.

On Thursday 10/29/20 at approximately 0541 hours while assigned to marked unit n499 I was dispatched to the area of 130 Vine Street for a report of a single vehicle roll-over crash. Upon arrival myself and n496(Ofc. Howes) observed a white SUV on its roof facing westbound in the eastbound lane of Vine Street (a public way owned, operated and maintained by the City of Newton). Upon our arrival Newton Firefighters had extracated the operator, (FOSTER Isabelle), who was sitting on a large rock in the front yard of 130 Vine Street. FOSTER was visibly upset, but didnt appear to have any major injuries. I asked FOSTER what happened and she stated she was travelling from Westgate Rd in Boston on her way home to Wellesley. She took a right turn onto Vine Street from LaGrange Street and began travelling westbound. FOSTER stated that she was travelling too fast and didnt realize how the road curved causing her to veer off the road and strike a large tree in the front of 130 Vine Street. FOSTER was travelling at a speed fast enough to cause the vehicle to flip onto its roof in the opposite lane of traffic. FOSTER admitted to me she was travelling too fast on the road. Vine street is a winding road that is not very well lit. Speed limit on the road is 20mph and its estimated FOSTER was travelling at least 30-45mph. The speed coupled with FOSTER's lack of familiarity with the area and her inexperience driving were all contributing factors to the crash. FOSTER was issued Massachusetts Uniform Citation T2012909 with the following civil motor vehicle infractions; C90 s18 SPEEDING (Posted) C89 s4a Marked Lanes Violation. FOSTER was transported to Beth Israel hospital by the medics and I was unable to issue the citation in hand at the time of the crash. Due to FOSTER's age (17) I also wanted to notify her parents and meet with them and FOSTER upon her release from the hospital. At approximately 1700 Hours I met with Scott Foster(Father) in person, Jodi Foster (Mother) and Isabelle foster via cell phone (speaker phone). I discussed the charges with all parties and explained the citation to Isabelle. Citation was given in hand to her father, Scott Foster and brough the citation to Isabelle.

Operator of vehicle one ( 2016 Infinity MA reg 3YD266 ) Miram Qualter, stated on 11/05/2020 at 10:28 hours while driving south bound on Waverley Ave near the intersection of Tremont St " the light was turning " as her vehicle entered the intersection. Both Tremont St and Waverly Ave are public ways in the City of Newton. Qualter stated that she was driving into solar glare and did not see any traffic in the intersection as she proceeded into it. Qualter stated that she believed that she still had the right of way and that traffic on Tremont St had a red light. Qualter stated that she did not see vehicle two until the time of impact . Qualter stated that she was not injured in the crash. Qualter's vehicle had major damage to the front driver's side windshield and front driver's side quarter panel. Operator of vehicle two Keith Ward stated that he was riding his Itraxx Cruze motor scooter west bound on Tremont St near Waverley Ave on 11/05/2020 at 10:28 hours. Ward stated that west bound traffic had a green light and he proceeded into the intersection. Ward stated that as he was approximately halfway into the intersection when vehicle one crashed into the right front of his vehicle. Ward stated that he was ejected from his motorcycle and onto the hood of vehicle one. Ward was wearing a helmet and his head had contact with vehicle one's driver side windshield causing substantial damage. Ward was evaluated by Newton Medic Unit 3 and refused medical treatment. Ward signed a patient refusal form provided by the medics. Ward had visible head lacerations that caused minor blood loss. Witness Jose David stated that he was riding on a motor scooter behind Ward. David stated that west bond traffic on Tremont St had a green light and that vehicle one did not stop at a red light. Based upon statements made to me and my observations of the involved vehicles, I issued Qualter MA Citation T2016100 and cited her for a violation of MGL 89/9 , failure to stop at a red light. I took photos of both vehicles and the disk was downloaded by the NPD's IT Bureau. Both vehicles were towed by Tody's Towing, and I filled out towed motor vehicle forms. During my investigation of this crash I discovered that the motor scooter that Ward was operating is classified as a motorcycle, because it's cylinder capacity exceeded 50cc. Ward's vehicle speedometer indicted that it's top speed was 50 MPH, and it's VIN tag stated it was a motorcycle. I observed that there was no license plate on Ward's vehicle and he stated to me that it was unregistered. A check of the MA RMV had no record of Ward's vehicle and also revealed that Ward's driver's license was suspended on 12/04/2018. It was also revealed from a of the MA RMV data base that witness Jose David's driver's license statue is expired non renewable as of 01/23/2017. David stated to me that he did not have a driver's license. I also discovered that David's motor scooter was classified as a motorcycle, because as with Ward's vehicle it also had a cylinder capacity exceeding 50cc. David stated to me that his vehicle was unregistered. I issued David MA Criminal Application T2015901 and cited him for violations of MGLs 90/10, operation of a motor vehicle with out a license, 90/34j uninsured motor vehicle and 90/9 , unregistered motor vehicle. I issued Ward MA Criminal Application T2015902 and cited him for violations of MGLs 90/23 , operating a motor vehicle after suspension, 90/34j, uninsured motor vehicle and 90/9 unregistered motor vehicle. I filled out an Operating After Suspension Affidavit and faxed it to the MA RMV Driver Control Unit.

On, 11/19/2020 at approximately 1700 hrs, Newton Police and Fire were dispatched to Commonwealth Ave. and Prince St. for a reported head on motor vehicle collision with injuries. Upon arrival, I observed MV#1 ( 1WDR15) facing eastbound in the westbound travel lane. MV#2 (986KW1) was also facing eastbound, but in the eastbound travel lane. Both vehicles had significant front end damage, consistent with a head on motor vehicle collision. The operator of MV#1 was sitting on the grassy median upon my arrival. She stated she was traveling eastbound on Commonwealth Ave. when she became distracted by the radio, crossed over the double yellow line into the westbound lane of Commonwealth Ave. and collided with MV #2. The operator of MV#2 was entrapped in her vehicle, but was able to be extracted from the vehicle by Newton Fire personnel. Operator #2 was able to inform me that she was traveling westbound on Commonwealth Ave. She noticed MV#1 cross over the double yellow line at a high rate of speed, and collide into her vehicle. The operator of MV#2 was conscious and alert and transported to Tufts Medical Center Boston for an evaluation. The operator of MV#1 declined medical attention and was given a Massachusetts Uniform Citation (T2014484) for violating MGL. Ch. 89. Sec. 4(a) Marked Lanes Violation. Both vehicles were towed away from the scene by Tody's Towing Service. Photographs of this collision were taken on the (N492) camera and will be attached to this report.

On 12/3/2020 at 1400hrs I investigated a past motor vehicle crash involving a pedestrian. The crash occurred on 12/03/2020 at approximately 1240hrs. The crash was located in the area of 483 Centre St. Centre St is a public way in the city of Newton. Vehicle 1 was traveling southbound on Centre St near the area of 483 Centre St. Pedestrian 1 was located on the southbound side of Centre St near 483 Centre St. Pedestrian entered the southbound lane of Centre St and was struck by Vehicle 1. As a result of the impact Pedestrian 1 sustained a minor injury to his left leg. Vehicle 1 had no known damage. No parties were transported. No tow was required. It should be noted that the crash was reported by the Operator of Vehicle 1 Daniel Zandman at approximately 1400hrs. Zandman states that he was operating his Vehicle southbound on Centre St just after the intersection at Church St. Zandman states that Pedestrian 1 unexpectedly ran in front of his vehicle. Zandman stated that he did not have enough reaction time to come to a complete stop and his front bumper made contact with Pedestrian 1. Zandman stated that after impact he immediately got out of his vehicle and checked on Pedestrian 1. Zandman described pedestrian on as a 10 year old white male identified as Dexter Jagoditish. Zandman stated that while on scene Dexter called his mother Jess Jagoditish who immediately responded to the scene. Zandman stated that he exchanged information with Jess Jagoditish who then took Dexter home. Zandman states that a witness Mia Martin was also on scene. At approximately 1425hrs I called and spoke to Dexter and his mother Jess Jagoditish. Dexter states that he was walking on the sidewalk near 483 Centre St. Dexter states that he wanted to cross Centre St and thought there were no cars coming. Dexter stated that he entered the southbound lane of Centre St and a car came out of no where and hit him. I then spoke to Jess Jagoditish who is Dexter's mother. Jess stated that she received a call from Dexter around 1240hrs stating he was just hit by a car. Jess stated that when she arrived on scene Dexter was with Zandman and a witness Mia Martin. Jess stated that both Zandman and Martin explained them what had happened. Jess stated that Dexter appeared to be ok so she decided to take him home. Jess states that at this time Dexter has a bruise on his left calf but otherwise appears to be ok. At 1551hrs I spoke with the witness Mia Martin. Martin states that just prior to the crash she was walking her dog behind Dexter. Martin states that "out of nowhere" Dexter sprinted into the southbound lane of Centre St. Mia states that she then observed Vehicle 1 attempt to slow down and swerve to the lest to avoid Dexter. Mia states that the passenger side front bumper then made contact with Dexter. Mia states at impact Vehicle 1 was traveling at a very slow speed. Mia states that Dexter was then knocked on to the ground but then immediately jumped up and started to run away. Mia states that both she and Zandman were able to get Dexter to comeback so that they could check to see if he was ok. Mia states that Dexter was then able to contact his mom who responded on scene. Due to this crash being reported after the fact, Fire, EMS or Police where not initially notified and did not respond to the scene. Pictures were also not taken of the crash scene. At this time this report is for documentation purposes. After speaking with all the parties involved it appears that the crash was caused by Pedestrian 1 entering the roadway in a manner which gave Vehicle 1 little or no time to react.

On 12/05/2020 at approximately 22:00 hours I was called in to respond to a single vehicle crash with serious injuries in the area of 291 Nahanton St. Nahanton St is a pubic way in the City of Newton. Upon my arrival at the crash scene with Sgt Devine I was briefed by Officers Daly and Anderson, ( see Ofc Daly's report for further information CAD # 20054728 in regard to changes take out against the operator of the truck ) I made the following observations of the crash scene, Nahanton St is a public way in the City of Newton that is a two way roadway divided by an unprotected merdium ( double yellow line ) that runs east to west. The roadway surface was wet due snow and ice. The roadway was plowed and the snow was pushed to the shoulder, althought wet the roadway was icy. There was approximately 2 inches of snow on the shoulders of the roadway. The road was light with overhead head street lights. The posted speed limit is 25 MPH. I observed a 2013 Chevrolet Silverado pick up truck that had major front end damage. The Chevrolet was at an angle with it's front end facing west bound and resting on the grass shoulder and into a wooded area. The Chevrolet rear was sticking out into the shoulder of the east bound lane of Nahanton St. The passenger side front and rear doors of the Chevrolet had been removed by the fire department to extricate the driver and passenger. The driver and passenger side air bags were deployed . The front passenger side windshield of the Chevrolet was completely missing and the driver's side had completely shattered. The front tires, hood, and engine had been compacted on it's self. The front and rear driver's side doors had been mechanically open by the fire department. The speedometer needle had stopped at 54 MPH. The Chevrolet left the roadway and left tire marks in the snow in the eastbound bike lane of Nahanton St. At a 106.4 feet from the beginning of the tire marks the first point of impact was with two tree stumps that were 3.5 feet from the roadway, four feet tall and 3.5 feet in diameter. The tire marks continue in the east bound bike lane and exit the roadway and onto a dirt shoulder at 136.1 feet from the beginning of the tire marks. The truck continued to the right of a utility pole and then it's passenger side made contact with a tree at 170.8 feet from the begin of the tire marks. The tree was 12 feet from the bike lane approximately 5 feet in diameter. The impact with the tree caused the truck to turn so that the rear was facing north east on the dirt shoulder and bike lane. The final point of rest of the rear passenger side tire was 200 feet from the beginning of the tire marks. The truck's rear diver side tire final point of rest was 209 feet from the beginning of the tire marks. The front driver's side tire was compacted into the trucks engine and the passenger side front tire was located behind the tree approximately 20 feet from the roadway. Sgt Devine took photos of the crash area and of the truck and the disk was download by the NPD's IT Bureau. The truck was towed from the crash scene by Todys's Towing to their lot for storage, I fill out a towed motor vehicle form. There were no reported witness who observed the crash. The crash was reported by an unknown passerby. On 12/07/2020 Ofc Gaudet faxed a request for preservation of medical records, blood and urine samples of Schneider to Beth Israel Deconess Medical Center.

On Saturday January 2, 2021 at approximately 0115 hours I was dispatched to 229 Auburn St for a motor vehicle accident into a tree. MV1 was operating eastbound on Auburn St when it crossed over the marked center lane. MV1 struck the westbound side curb and jumped onto the sidewalk. MV1 then continued on the sidewalk until it struck the tree in front of 229 Auburn St. MV1 sustained total front end damage, as well as driver side damage. The driver side damage was to both driver side doors and both driver side tires were flat. MV1 was towed by Todys. MV1 operator was injured but conscious. However MV1 operator was unable to speak due to possible concussion injuries. MV1 was transported by Newton medics to Newton-Wellesley Hospital. MV1 was issued MA Uniform Citation T2015088 for marked lanes and speeding over reasonable and proper rate, and it will be sent to the MV1 operator via mail.

IT IS UNKNOWN AT THIS TIME IF THE VEHICLES IN THE DRIVEWAY HAD DAMAGE FROM THE CRASH DUE TO MUD AND GRASS ALL OVER THE VEHICLES. OWNER OF VEHICLE #1 REPORTED NO INJURIES. VEHICLE #1 WAS TOWED BY TODYS. On 3/31/21 I was working N503 when I responded to 68 Brierfield Rd. for a report of a past hit and run MVA. Upon arrival I spoke with the rp Eli Weisberger (the operator of Veh 2) and his brother Leo who stated that they were traveling westbound on Dedham St. when a car backed out from 98 Winchester (in front of 5 Dedham St.) colliding with their vehicle. Eli stated he saw the vehicle backing out and came to a complete stop and the vehicle backed right into him. Eli stated the operator of Veh 1 got out of his car and asked if they were ok to which they stated they were. Eli then stated the man said you know me I work at 711 and then drove away. I then spoke with Salim Grereige the operator of Veh 1. Salim stated he was backing out of 98 Winchester St. when he collided with Veh 2. Salim stated he looked both ways before he backed out and thought it was clear. Salim believed either Veh 2 was speeding or had just pulled out of the park right next to the parking lot. Salim stated they both got out of the cars to examine the damage. Salim said both parties agreed their was minor damage and Salim stated if you need me I work at 711. Salim stated he knows Eli from he being a consistent customer their. Salim stated they never asked for any of his information. It should be noted this accident occurred around 330pm and was reported at around 6pm. All parties stated they were uninjured. Both parties were provided an accident report number.

Operator #1 states that she was traveling northbound by 226 Lexington St when she observed vehicle #2, a white Ford Explorer, approaching at a high rate of speed from Lexington St southbound. Vehicle #2 then crossed the double yellow line into the northbound lane, subsequently sideswiping vehicle #1. Operator #1 states that two parties exited the Explorer and spoke briefly to her. Once she requested to exchange information, the two refused and drove away on Lexington St southbound. Operator #1 described the parties as a hispanic male and hispanic female, around 50 years old. The male party was balding/or bald, and the female was wearing a silver turtleneck. Operator #1 noted that it was the male who was operating the vehicle both during the collision and as the vehicle left the scene. While medics were documenting a patient refusal for operator #1, Ofc Ciccone was en route to NPD headquarters to report for duty in his p/v and heard the broadcast for the suspect vehicle. Ofc Ciccone observed a white Explorer with a flat tire traveling an estimated 50+ mph past him on South Ave by Park Rd in Weston heading towards Natick. This location is approximately 2.2 miles from the crash site and is accessible from 226 Lexington St by heading southbound, taking a right, and continuing straight on Rt 30. Weston Pd was immediately notified and the vehicle was located in a driveway at 7 High Meadow Rd, a residence directly on the corner of Rt 30. Sgt Rooney and Ofc Ciccone (who was able to take a spare cruiser and call into service as this incident unfolded) responded and met Weston PD. While on scene, it was explained to all officers by the resident of 7 High Meadow Rd, Helder Quesada, that he was walking his dog and observed the white Explorer broken down in an unsafe spot. Quesada offered up his driveway to park the vehicle as the male and female occupants waited for assistance. Quesada allegedly does not know Lopez and Albizureis, and all involved persons were outside in the driveway when police located the vehicle. The description of the male asociated with the Explorer matched that which operator #1 gave to me- a hispanic male balding in his 50's. He was in the presence of a hispanic female who was wearing a silver colored turtleck. The male was identified as Armando Lopez-Rubi DOB 1/1/73 and the female Ana Maria Albizureis DOB 11/27/73. The Explorer was MA reg 6GZ591, registered to a different party then all on scene. It had obvious damage to the left side doors consistent with an opposite direction sideswipe. With the translation assistance of Helder Quesada, Lopez-Rubi stated to Sgt Rooney and Officer Ciccone that he was in fact driving somewhere near Starbucks "on the corner" when he got into an accident (there is a large Starbucks located on the corner of Commonwealth Ave and Lexington St in Newton). Lopez originally said that the other involved vehicle left the scene, however it was pointed out by Sgt Rooney that the operator was currently stopped in Newton talking to me and was cooperative. Upon routine query it was discovered that Lopez-Rubi does not have a license to operate a motor vehicle. Pursuant to the investigation, Officer Ciccone issued in hand to Lopez MA citation R8006998 for the following: 89/4a- Marked Lanes Violation 90/24- Leaving Scene of Property Damage 90/10- Unlicensed Operation of a Motor Vehicle. Vehicle #2 was left in the driveway of 7 High Meadow Rd with Helder Quesada's permission. Quesada also volunteered to drive home both Lopez and Albizureis to Framingham, as he was feeling charitable. Vehicle #1 was towed by Tody's due to the amount of damage and airbag deployment. A m/v inventory form was filled out.

On Sunday June 13, 2021 at approximately 1933 hours I was dispatched to 2322 Washington for a two car MVA with no injuries. MV1 operator stated she was operating eastbound on Washington St (a public way) driving the speed limit. MV1 operator stated she observed MV2 on the westbound side of Washington St attempt to turn left into Starbucks (2322 Washington St). MV1 operator stated by the time she saw MV2 cut across in front of her and into her lane, it was too late and MV1 struck the passenger side of MV2. MV2 operator stated the same story. MV2 operator said she was also going the speed limit on Washington St westbound and attempted to turn left into Starbucks but misjudged MV1's speed, causing the MVA. MV1 sustained extensive total front end damage and was towed by Tody's. MV1 operator requested an evaluation by Newton Medics and was transported to Newton-Wellesley Hospital. On 06/14/21 at approximately 12:00 hours, Sgt. Wade received an email from Nina Wang (D.P.W. Transportation Traffic Superintendent) that a city utility trailer with a signboard attached to it was struck by another motor vehicle at 275 Brookline Street sometime over the weekend between 06/11/21 and 06/14/21. A photo was sent over in the email of the utility trailer and signboard knocked over on it's side over the sidewalk. The utility trailer is 2020 Wanco color orange bearing MA Official registration # M9994A. The trailer was parked in the bicycle lane of the northbound travel lane next to 275 Brookline Street. The signboard was there in an attempt to get motorists to drive the speed limit due to the Memorial Spalding School located at 250 Brookline Street. On 06/14/21 at approximately 12:00 hours, I responded to Brookline Street to photograph the trailer and possibly process the scene. The trailer could not be located. I learned later that the trailer had already been towed by Tody's towing and brought to the City D.P.W. yard located at 110 Crafts Street. On 06/15/21 at approximately 09:00 hours, I went over to 110 Crafts Street and was told that the trailer/signboard was in the garage. The trailer was already being repaired to be placed back in to service. It had sustained damage to the two rear brackets where the jacks are located and the license plate bracket and registration plate were bent. The signboard message frame board is no longer aligned properly as a result of the accident but reportedly still functions. After, I then responded back to 275 Brookline Street and took digital photos of the scene. I observed in the bike lane at the accident scene two small pieces of lens covers to a vehicle with no identifying marks. I also did a canvass for possible surveillance cameras in the area. There was no response at number 269 and 275 Brookline Street but I left a note for them to call me. 283 Brookline Street has no surveillance cameras. Memorial Spalding School is across the street from the accident and I observed a camera on the side of the building which faces the staff parking lot on to Brookline Street. I spoke to a staff member Jesse Brackett at the school. He called David Stickney (Superintendent of Schools) and notified him of the accident. Mr. Stickney was provided with my email and will send over any possible footage if he has it. At approximately 12:45 hours, I spoke to Mike Sylvester who resides at 275 Brookline Street. He stated that he had surveillance footage and emailed it over to me. I viewed the footage which is a camera that views his approx. 75 foot driveway leading out to Brookline Street. There is a fence and bushes behind where the signboard is located which limits it's view. The video shows the accident took place on 06/11/21 at 18:48 hours. At this time, you can see the trailer is struck and slowly falls over on it's side. Immediately after a white colored four door sedan continues to slowly drive past the scene northbound on Brookline Street in the bicycle lane without stopping. Two other vehicles behind the white sedan drive around to the left of the white vehicle at this point. In the footage, you can not view the registration number, operator details, or the type of sedan from the footage. On 06/16/21 at 09:40 hours, I spoke to David Stickney who called the Traffic Bureau. He stated that he had footage of the accident but it did not reveal any other details besides the same white sedan striking the trailer.

Witnesses #1 and #2 (anonymous) stated that Vehicle #1 was parked in a parking space behind Pressed Cafe. Vehicle #2 was parked to the right of Vehicle #1. Vehicle #2 attempted to back up out of their space, overcorrected, and backed into Vehicle #1. Witnesses stated Vehicle #2 drove away at a high rate of speed. Vehicle #1 suffered minor damage to the right rear side of the vehicle. Unknown damage to Vehicle #2. Witnesses followed the vehicle and provided me with a picture of the license plate. They were unable to describe the driver. I was able to locate the owner of Vehicle #1 and notify him of the damage. I responded to the address listed for Vehicle #2 with negative results. Report forwarded to Traffic Bureau for follow up with Vehicle #2. Traffic Bureau update (Officer Gaudet): On Tuesday, July 6, 2021, I made contact with the registered owner of MV2, Ms. Irina Mazo via telephone. Ms. Mazo stated she was away for the weekend but her daughter, Olga Rapoport, had the vehicle. I apprised Ms. Mazo about the crash involving her vehicle. Ms. Mazo stated she would contact her daughter and have her call me. A short time later, I received a call from Ms. Rapoport. Ms. Rapoport stated she was operating MA: 4LD211 at the time of the reported crash. Ms. Rapoport stated her rear passenger side bumber area crashed into MV1 as she was backing out of a parking spot. Ms. Rapoport stated she left the area with out stopping to assess the damage to MV1, or to attempt to make contact with the owner of MV1. I asked Ms. Rapoport why she did not stop. Ms. Rapoport stated she did not have an excuse. Ms. Rapoport stated there is no damage to her vehicle. Ms. Rapoport was advised and will be mailed Massachusetts Uniform Citation (061166AB) for Chapter 90, Section 24 (Leaving the Scene of Property Damage).

On Monday, July 19, 2021, while assigned to Traffic unit N525, I responded to the area of Commonwealth Avenue and Lowell Avenue, Newton for a report of a MVA/Pedestrian crash. Commonwealth Avenue and Lowell Avenue are both public ways maintained by the City of Newton. The weather at the time of the crash was overcast with light rain. The road surface at the time of the crash was wet. Feeney Brothers Utilities has been performing gas repair services at this intersection for the past month and they have had a paid Newton Police Detail with them during their service. The detail presented itself as follows on my arrival. There was service being conducted in the northbound lane of Lowell Avenue in-between the Commonwealth Avenue carriage roads. A large trench was present in the northbound lane that made the roadway impassible. Traffic was permitted to travel down the southbound lane of Lowell Avenue towards Commonwealth Avenue for the detail. Access to Lowell Avenue from Commonwealth Avenue was prohibited. The northbound lane was blocked by orange construction cones and road closed signs. There was also no access to the Commonwealth Avenue carriage roads at this location. Both of these roadways were closed with orange construction cones and caution tape. There was also caution tape and orange construction cones restricting access to a walkway that traveled from Commonwealth Avenue to its carriage road just after Lowell Avenue (E). I also observed a "Sidewalk Closed" sign posted on the walkway. I spoke with Officer Gabriel of the Newton Police Department. Officer Gabriel stated he was working a paid detail for Feeney Brothers at this location. Officer Gabriel stated he had just assisted two female parties cross Commonwealth Avenue (E) at Lowell Avenue. Officer Gabriel stated a short time later, he heard a scream and observed two female parties laying on the roadway on the opposite side of Commonwealth Avenue in the carriage road. Officer Gabriel immediately contacted Newton Police Dispatch via his portable radio and requested aid for the injured parties. Prior to my arrival, Officer Gabriel stated one of the pedestrians was transported to Newton Wellesley Hospital with a head injury. I spoke with one of the pedestrians involved in the crash, Ms. April Stein. Ms. Stein stated she was walking in the carriage road of Commonwealth Avenue (E) towards Lowell Avenue with her friend Sarah Kish. Ms. Stein stated when they got to Lowell Avenue there was work being done in the roadway, and a Police Officer assisted them with crossing the area. Ms. Stein stated they then re-entered the carriage road when they crossed Lowell Avenue. Ms. Stein stated they were walking along the edge of the roadway when they were hit by a front loader. Ms. Stein stated they fell to the roadway due to the collision. Ms. Stein stated she did not hear or see the front loader backing up towards them and did not notice it because they were having an "intense conversation". Ms. Stein reported no injuries and signed a patient refusal with Fallon Medics. I asked Ms. Stein if she had transportation to her residence. Ms. Stein stated she would walk home. I spoke with the operator of the front loader involved in the crash, Mr. Mark Mannion (S91049325). Mr. Mannion is an employee of Feeney Brothers Utilities and stated he was operating a 2015 Case Loader (MA CON: R99118) at the time of the crash. Mr. Mannion stated he was backing up the loader down the Commonwealth Avenue carriage road (E) from Lowell Avenue. Mr. Mannion stated he was going to get a load of fill from a pile of dirt on the side of the road inside the marked off work area. Mr. Mannion stated as he was backing up, he was looking over his right should behind him to make sure there were no obstacles in his way. Mr. Mannion stated his emergency backing alarm was activated while he was backing up. Mr. Mannion stated he noticed something enter the roadway to his right side and immediately slammed on his brake. Mr. Mannion stated then heard a scream and saw 2 female parties on the ground. Mr. Mannion stated he exited the loader and checked on the welfare of the two women. Mr. Mannion's CDL License, Vehicle Registration, and Hoisting License are all active. I asked Mr. Mannion to put the vehicle in reverse to assess if the emergency backing alarm was functioning properly. Mr. Mannion was cooperative and while reversing the vehicle the emergency backing alarm was functioning properly in my presence. I asked Mr. Mannion if the loader had moved since it was brought to a stop due to the crash. Mr. Mannions stated it had not moved. I observed this vehicle to be within the parameters of the work zone that was taped off. The vehicle at this time was released back to Feeney Brothers. Photos were taken of the work site and loader and submitted to the IT Bureau. I responded to Newton Wellesley Hospital and spoke with Ms. Sarah Kish (S85088490) in the Emergency Room. Ms. Kish stated she was walking down the Commonwealth Avenue carriage road (E) towards Lowell Avenue with her friend April Stein. Ms. Kish stated a Police Officer assisted them with crossing Lowell Avenue via the crosswalk located on Commonwealth Avenue. Ms. Kish stated after they crossed Lowell Avenue, they walked to the carriage road. Ms. Kish stated as they were walking, she was hit from behind by a front loader in the roadway. Ms. Kish stated she did not hear or see the front loader and believes the bucket hit her in the head. Ms. Kish stated she fell to the roadway. Ms. Kish was transported to Newton Wellesley Hospital for her injuries. Ms. Kish stated at this moment, she had some lacerations to her back and head, and her left shoulder/neck/back area was sore. I asked Ms. Kish how often she walks that particular route. Ms. Kish stated 5 times a week. Ms. Kish stated there was nothing in place to restrict her access down to the carriage road after she crossed Lowell Avenue. I spoke with a witness to the crash via telephone, Ms. Megan Boitano. Ms. Boitano stated she was traveling on Lowell Avenue (N) towards Commonwealth Avenue. Ms. Boitano stated she was stopped at the light right next to the trench that was in the northbound lane of Lowell Avenue. Ms. Boitano stated she observed two women crossing Lowell Avenue at Commonwealth Avenue in front of her. Ms. Boitano stated she then observed a heavy piece of construction equipment approaching the trench to her left at a high rate of speed. Ms. Boitano stated it startled her as the front loader dumped dirt into the trench. Ms. Boitano stated she then observed the front loader back down the carriage road at a high rate of speed towards a dirt pile. Ms. Boitano stated she observed the two women that just crossed Lowell Avenue in the roadway behind the front loader. Ms. Boitano stated she lost sight of the women, and then observed the front portion of the front loader aggressively shake. Ms. Boitano stated she then observed one of the women walk around from the side of the front loader holding her head. As presented while investigating this crash, the Feeny Brothers work site appeared to be properly set up to mitigate a potential safety incident with ample cones, barriers, caution tape, and signage. There was also a police detail present to assist with pedestrian travel. The area of the work zone where the pedestrans entered from Commonwealth Avenue after crossing Lowell Avenue was taped off with caution tape and a sidewalk closed sign was present. The crash happened inside the work zone parameters.

Mv#1 operator stated she was travelling straight ahead on Beacon St E/B (right of way) in the area of #716 Beacon. On Wednesday, September 1, 2021, while assigned to Traffic unit N525, I responded to the Newton Centre Municipal Parking Lot (797 Beacon Street, Newton) for a report of a motor vehicle crash involving a pedestrian with a potential head injury. The weather at the time of the crash was cloudy and overcast. The road surface at the time of the crash was dry. The Newton Centre Municipal Parking Lot is open for public travel and use and is maintained by the City of Newton I spoke with the operator of the 2016 Audi 32 (MA: 4PL345) involved in the crash, Mr. Peter Schiller (S26414610). Mr. Schiller stated he entered the Newton Centre lot from Langley Road and made a right turn to enter the back row of parking stalls. Mr. Schiller stated he then took a left turn into the parking area to look for a spot. Mr. Schiller stated he did not see the pedestrian in the roadway in time and his vehicle's front end crashed into the pedestrian. Mr. Schiller estimated the crash happened right near the first car parked in the row closest to where he entered. I showed Mr. Schiller the believed area of impact was actually 3 car stalls in, and the crash occurred in the middle of the travel way. I asked Mr. Schiller if he has any medical conditions or takes any medication that would effect his ability to operator a motor vehicle. Mr. Schiller stated he does not. Mr. Schiller reported no injuries as a result of the crash. I observed very minor damage to the front hood/bumper/fender area of his vehicle from where the crash took place. I did observe soil covering the hood area. The injured pedestrian was carrying a plant with soil in the pot. I asked Mr. Schiller if he could recall where he made contact with the pedestrian or how the pedestrian ended up in front of his vehicle. Mr. Schiller stated he thought the injured pedestrian was running towards his car when the made his turn. I spoke with a witness that remained on scene, Mr. Nicholas Ryland. Mr. Ryland stated he was walking in the Newton Centre lot while pushing his daughter in her stroller. Mr. Ryland stated he observed MV1 enter the lot from Langley Road and took the right turn into the lot at a high rate of speed. Mr. Ryland stated he stopped with the stroller next to the first vehicle parked in the roadway where the last parking stalls begin. The MV1's driver side operated past Mr. Ryland's left side, took a left turn, and then Mr. Ryland stated he heard the crash. The crash took place in the middle of the roadway in between the parking stalls to the East and West of MV1. I responded to Brigham and Women's Hospital in Boston to follow up with the pedestrian injured in the crash, Ms. Sophia Alexandridies (S49399467). Ms. Alexandridies was transported to the emergency department by Fallon Medics with multiple injuries including a head injury. Due to Ms. Alexandridies's injuries, I was unable to obtain a statement from her about the events of the crash. I spoke with Ms. Alexandridies's daughter, Christina, in the emergency department. Christina stated Ms. Alexandridies suffered a head injury, along with possible broken ribs and an injury to her left shoulder. Most of the injuries reported are on Ms. Alexandridies's left side, the presumed area of impact with the vehicle. Due to the circumstances of this crash, and Mr. Schiller being involved in another surchargeable crash on April 28, 2021, a Request for Immediate Threat License Suspension was submitted to the Registry of Motor Vehicles at DCUImmediateThreat@massmail.state.ma.us. Mr. Schiller is also being cited with Massachusetts Uniform Citation 119853AB for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). On September 2, 2021, I contacted Mr. Schiller to provide him with additional information about the crash. Mr. Schiller stated he did not realize Ms. Alexandridies was injuried in the crash. Mr. Schiller also stated he has no recollection of being involved in a crash in April. Mr. Schiller was advised to contact his insurance company to inquire about his last crash. Photos were taken of the area and MV1 and submitted to the IT Bureau. I canvassed the area for survellance footage of the crash with a negative result.

On 10/02/2021 while assigned to marked cruiser N503 at approximately 20:46 hours I, Officer Vazhenin, responded to 601 Washington Street for a report of 2-car motor vehicle accident. Upon arrival on scene, I spoke to Mr. Jeffrey Yanfeng Xiao (Driver 1), who stated that he was driving eastbound on Washington Street when he observed another car (Vehicle 2) pulling out of 7/11 parking lot at 612 Washington street. Driver 1 said that in order to avoid collision with Vehicle 2, he had to swerve to the left in oncoming traffic lane crossing double yellow line. He said that while he was passing Vehicle 2, Vehicle 2 kept driving and crashed into the passenger side of his car (Vehicle 1). I then spoke to Ms. Shahrbanou Asadi, the operator of Vehicle 2, who stated that she was pulling out of 7/11 parking lot and was trying to make a left turn on Washington street. She stated that Vehicle 1 "showed up out of nowhere" and was traveling at high rate of speed. Ms. Asadi stated that she proceeded making her left turn when Vehicle 1 suddenly appeared in the front of her car and her car (Vehicle 2) crashed into passenger side of Vehicle 1. It should be noted that Ms. Asadi did not have her driver's license in possession. License plate inquiry of her car (Vehicle 2, MA Reg.#883MH8) revealed the fact that her driver's license was expired, which she failed to disclose to me when asked about status of her registration and driver's license. I ussued a Criminal Application to Ms. Shahrbanou Asadi on Massachusetts Uniform Citation #T1445734 for Ch.90 S.10 "Unlicensed Operation of Motor Vehicle", Ch.89 S.8 "Failure to yield", Ch.90 S.11 "License not in possession".

On 10/7/21 at about 1145am I was dispatched to 1101 Beacon St the Walgreen's parking lot for an MVA with injuries. On arrival, I observed 3 parked unoccupied MVs heavily damaged that were smashed into each other. I was approached by one of the MV owners, Tony Dooley. Mr Dooley stated he was walking out of the Walgreen's front door heading to his parked MV (#3) which was approximately <30 feet away. His attention was suddenly drawn to a speeding MV (MA reg 3MKH84) driving through the lot that veered right and smashed into the 3 cars. Mr Dooley's MV was the middle car smashed between the other 2. The female operator briefly stopped afterwards and pulled into an adjacent parking spot at a crooked angle. I approached the female operator who was being taken out of her car and placed onto the stretcher by medics for transport. Female operator was identified as Ms Norma Greenberg. I asked her what happened? She replied in a bewildered manner "I have no idea." I asked her if she was taking any medications today and she replied yes, to treat a spinal condition she has. Ms Greenberg had lacerations on her legs and arm with heavy bleeding as a result of the impact.Ms Greenberg's vehicle (#1) was towed with significant front damage and airbag deployment. MV#2, the first car struck on impact, was towed with very heavy passenger side, rear end damage and driver's side damage. MV#3, the middle vehicle, was towed with significant passenger side damage, flat tire and driver's side damage. MV#4, sustained significant passenger side damage which was able to be driven from the scene. Based on observations and statements, it appeared Ms Greenberg had suddenly dangerously accelerated through a congested parking lot causing heavy damage. Ms Greenberg's driving history shows surchargable accidents from 7/1/21, 11/1/20, 8/25/15 and 12/26/10. Due to Ms Greenberg's inability to recall how or what happened, I am seeking an Immediate Threat application due to and not limited to medical reasons to suspend Ms Greenberg's license through the RMV.

Sagar Mohan stated he and his son were in the park at the time of the accident and they saw a red sedan going east bound at a high rated of speed with no concern to thee narrowness of the street and the tight turn. They stated it looked like operator 1 was trying to avoid a collision when he swerved into the fence. Unit 523 took several digital images of the fence and parks and rec were notified.

On October 26th 2021 while assigned to the front desk, Hanna Yoon DOB 9/9/83 came in to report a minor past mva/pedestrian. Yoon states that around 0820 hrs today, she was pulling out her driveway at 357 Langley Rd (Ma reg 5HL884). Yoon did not see a pedestrian approach from the right, and contact was made at a slow rate of speed. Yoon states the pedestrian did not fall down. She believes that the front right part of the vehicle hit the person's left thigh. Yoon asked the pedestrian if she was ok, to which the woman stated she was fine. The woman asked for and was provided Yoon's cell phone number before walking away. Her clothing description is unknown. Around 1100hrs Yoon then received a text from 617-913-5141. The person texting identified them self as the woman who was hit, and stated she was in the hospital with leg pain. She then texted that the hospital would contact Yoon for more information. Yoon did not respond and has not been contacted by anyone since this text. A name was not provided by the texter. There are no entries in QED indicating that the unknown pedestrian has called to report this accident. I tried to call the number provided with negative results. At this time, there is no identifiable victim. Yoon was advised that I will document her statement, and in the future it is best to call police to the scene of a crash in order to avoid confusion. Update by Lt. Dangelo: On 10/27/21 at approximately 0830 hrs. the pedestrian from the accident, Woo Jin Seo came into HQ with her mother to report the accident. She stated that on 10/26/21 at approximately 0820 hrs. she was walking to Newton South High School from 383 Langley rd. when a vehicle backing out of a nearby driveway bumped her. She stated that she did not fall down, but did suffer an injury to her leg. She stated that the operator of the vehicle stopped and checked and left her phone number. Ms. Seo went to Beth Israel Lahey urgent care the same day for leg and shoulder pain. I contacted the operator Hanna Yoon and updated her.

On November 21st, 2021 at approximately 07:56 hours while working N491 I along with Officer Mike Boudreau responded with NFD and Medic One to a report of a MV crash with injuries in the area of Centre St @ Franklin St. On my arrival the Medics were evaluating two adult males who standing outside one of the involved vehicle from this crash. There appeared to be several involved vehicles to this crash however two of them had been parked prior to the crash. The vehicle which the two unidentified males were standing by was MA reg. 9HY939, a 1988 Black Ford F150 PU, listed registered owner was a Peter J. Jara-Castro, #28 Ridge Ave, Lynn Ma. Mr Castro was one of the two males standing by that truck. The above vehicle appeared to have been travelling N/B on Centre St when it skidded off the road, jumping the curb striking a 1 foot slate retaining wall to the property to #573 Centre St. It continued onto that property driving over shrubs, small trees, then onto the driveway(same address) which is actually on Newtonville Ave, hitting the homeowners 2007 Nissan Versa, Ma reg. 6BH269, forcing it into more shrubbery. The Truck continued across Newtonville Ave where it struck another parked vehicle, a Grey 2019 Volkswagon, Ma reg. 1NFC27 on the rear passenger side pushing it forward. The truck finally came to a stop after driving into a wooden fence to the property of #559 Centre St.(Fence is on Newtonville Ave.) I spoke with the owner of the truck after he was evaluated by the Medics, who spoke limited English. The owner of the truck Peter Jara-Castro finally said he was driving the truck prior to the crash. His statement was he was travelling N/B down Centre St when another vehicle came out from Newtonville Ave causing him to slam on his brakes. Mr Castro doesn't remember much after that, possibly from hitting his head. At this time I couldn't determine who the actual driver was. Both Mr Castro and the passenger, sustained injuries but both declined to go to the hospital. Both were later picked up by an UBER driver. A Firefighter heading to work reported seeing the truck pass him on Centre St N/B at high rate of speed but never saw the driver or the actual crash. Both property owners that sustained damage were given Trucks insurance info. Pictures were taken by both property owners. Vehicle #1 and vehicle #2 were towed from scene. Both sustained heavy damage. Vehicle #3 sustained minor damage I was unable to make contact with that owner at this time. However later in the day I was able to make contact with owner of vehicle #3. She was given all vehicle #1 information along with his insurance. A citation for speeding was later issued after investigation was completed and witness statement. Citation#t1444076 was later sent VIA mail.

On Sunday November 21st at approx. 1720 Hrs. I responded to 191 Jackson Rd for a motor vehicle accident roll over with entrapment. Upon arrival I witnessed MV1 on its side in the middle of Jackson Rd. The operator of MV1 was conscious and alert and was taken out of the MV by the Newton Fire Department. The operator of MV1 stated that she was driving S/B on Jackson Rd. and she did not see the parked car on the right side of the road. MV1 then tried to swerve around MV2 and clipped the left rear bumper, and left rear door of MV2 which caused MV1 to roll over onto the middle of the road. MV1 had major damage all around the vehicle and all airbags deployed. MV2 had major damage to the rear and to the left side of the MV. Due to the extensive damage and length of travel of MV1 it was estimated that MV1 was traveling at a speed greater than 35 MPH. Speed limit for Jackson Rd is 25 MPH. MV2 was pushed approx. 20 feet from where it was parked, and MV1 travelled approx 25 feet after on its drivers side after impact. The operator of MV1 was transported by EMS to NWH, and Todys's took possession of both motor vehicles. Pictures were taken and submitted to IT. The operator of MV1 was cited 90/17, speeding.

At approximately 1425HRs on Wednesday, November 24, 2021, I was dispatched to a motor vehicle crash, which took place on Nahanton St., between number 293 and Winchester St. Upon arrival, I observed MV1 to be facing eastbound in the westbound travel lane. It had its passenger's side front bumper contacting the passenger's side front bumper of MV3 and MV2 was off of the roadway, to the left of MV1's front driver's side. MV1: I noted damage to its front driver's side bumper, head light, fender, hood area and flat tire. On MV1's front passenger's side, I noted damage to its bumper, head light, fender and hood area. MV2: I noted damage to its front driver's side bumper, head light, fender, hood area and flat tire. MV3: I noted damage to its front passenger's side bumper. Upon speaking with the operator of MV1, they stated they were traveling eastbound on Nahanton St. At this time, they began to retrieve something from their bag on the passenger's seat. They then crossed MV 1 over the center yellow divider lines and struck MV2 and MV3. Upon speaking with the operator of MV2, they stated the last thing they remember is that they were traveling westbound on Nahanton St. and then that their vehicle was traveling backwards; They do not remember the crash. Upon speaking with the operator of MV3, they stated they were traveling westbound on Nahanton St. when MV1 crossed over the center yellow divider lines at a high rate of speed. They observe MV1 crash into MV2, pushing it backwards and to the side. MV1 then continued forward and impacted MV3. The operator of MV1 received a ride from their father, after signing a patient refusal. Newton Medics transported the operator of MV2 to the hospital. And the operator of MV3 was able to drive their vehicle away, after signing a patient refusal. Both MV1 and MV2 were removed by the scene by Todys Services (NPD Towed Motor Vehicle Form Filled). I cleared without further incident.

On Wednesday 12/1/21 at approximately 0800 hours while assigned to marked unit n498 I was dispatched to 336 Winchester Street for a report of a single vehicle crash with no injuries. Upon arrival I observed a white BMW X3 partially off the road with heavy front end damage on the Northbound side of Winchester Street in front of 336. I spoke with the operator, PHILLIPS Illana, who states she was travelling Northbound on Winchester Street at approximately 30 MPH when she heard a pop and the car pulled immediately to the right and she felt an impact and the air bags deployed. It appears as though the impact she felt was the Utility Pole in front of 336 Winchester Street. Pole sustained significant damage, but still appeared to be safe. Verizon was notified to respond as soon as possible. Also on scene was STOLYAR Polina, who states that she was walking on the sidewalk toward her house at 345 Winchester Street. Polina states she observed the white BMW travelling at a normal speed Northbound and she also heard a pop and saw a quick flash that she described as a flame come from the passenger side tire of the vehicle causing it to swerve off the road and strike the Utility Pole. Polina stayed on scene to make sure PHILLIPS was ok and to speak with me. After speaking with the operator, PHILLIPS, and the witness, STOLYAR, it appears to me that the passenger side tire of the BMW blew causing the vehicle to veer off the road striking the Utility Pole. I walked Southbound down the Northbound side of Winchester Street to see if I could locate any sort of road defect that could have caused the tire to suffer a catastrophic malfunction. I was unable to locate any such defect. PHILLIPS mentioned to me that recently the light on her dashboard illuminated indicating low tire pressure and she brought the vehicle to have the air pressure refilled. She expressed a concern in hindsight that whomever added air may have over inflated the tire. Exact cause of tire malfunction is unknown at this time.

Operator of vehicle one, Elizabeth Oppenheim stated that while driving her 2021 Subaru Crosstrek SUV ( MA reg 72GL56 ) west bound on Watertown St in front of 392, a pedestrian stepped off the south side walk and into the roadway. Oppenheim stated that she did not see the pedestrian due to low light conditions. Oppenheim estimated that her vehicle's speed at the time of the crash to be 10 to 15 MPH due to heavy traffic. Oppenheim stated that the pedestrian latter identified as Joseph Colucci stepped off the sidewalk and into her path of travel in the roadway. Oppenheim stated Colucci was not in a marked crosswalk. Oppenheim stated that she stopped her vehicle to check on Colucci who told her that he was not injured and did not want medical aid. Colucci stated that he was struck on his right side and fell to the ground. Colucci signed a patient refusal form provided to him by medics. Colucci stated that he did step off the side walk in front of 392 Watertown St to cross the street. Colucci acknowledged that he was not in a marked crosswalk at the time of the crash. Watertown St is a pubic way in then City of Newton.

Witnesses at the scene informed me that MV#1 was traveling west on Commonwealth Ave in the area of #2102 when MV#2, who was traveling east on Commonwealth Ave in the same area crossed the yellow line and crashed into MV#1. The operator of MV#1 was upset and sitting in MV#1 when I interviewed her and only stated that her left hip hurt. Operator #1 refused medical treatment at that time and wanted to wait for her mother to arrive before making any decisions. The operator of MV#2 stated that he was traveling east on Commonwealth Ave in the area of #2102 in the left hand lane when a pedestrian appeared in the roadway. According to operator #2, he cut his steering wheel to the left to avoid the pedestrian and struck MV#1. No tows and only a minor injury to the operator of MV#1, though she did not want to be evaluated by the medics. Two independent witnesses to the crash gave me similar statements that operator #2 had driven on the wrong side of the road prior to the crash and was operating at a high rate of speed. Operator #2 cited for failure to obey signage. Only after I completed this report, was I notified by Cheryl Petitti, the mother of Marisa Petitti that Marisa was not operating MV#1 at the time of the crash and that she may have been struck by MV#2. I re-interviewed both witnesses and they confirmed that Marisa was in fact the pedestrian crossing the street, which I did not know, but they did not witness her being struck by MV#2 due to their view being obstructed. Cheryl Petitti also states that she was contacted by another witness, who had left prior to my arrival, that has information to provide. At this time I will notify the Traffic Bureau of a possible Auto Pedestrian to be further investigated.

On Monday, December 06, 2021, while assigned to Traffic unit N525, I responded to the area of Midland Avenue and Riverdale Avenue, Newton for a report of a motor vehicle crash resulting from a medical event. The weather at the time of this incident was overcast. The road surface in the area of the incident was mostly dry. Midland Avenue and Riverdale Avenue are both public ways in the City of Newton. I spoke with Mr. William Shea (S45354021), a foreman for Diaz Construction who was working on a job site next to 10 Midland Avenue at the time of the incident. Mr. Diaz stated while working with Mr. Silvestre Laneiro (S45163124) and Mr. Leonardo Alvarenga (S63414490), they observed a green Ford Taurus (MA: 6BF469) traveling towards the area of their job site while traveling North on Riverdale Avenue. Mr. Shea stated he observed a white male party vomiting what appeared to be blood out of the driver side window of the vehicle. Mr. Shea stated he then observed the vehicle's driver side area crash into numerous cement posts in front of 10 Midland Avenue (PSMJ Resources, Inc.) at a low rate of speed. The vehicle then traveling over a large mental plate in the roadway covering a trench and it continued down Riverdale Avenue until it crashed into the front driver side tire of a parked 1998 Volvo 828 rock truck owned by Diaz Construction. This vehicle was parked unoccupied on the roadway inside the jobsite. Mr. Shea stated they all went over to the vehicle to assist the driver, later identified as Mr. Richard Proia (S25282466). Mr. Shea stated there was a large amount of blood all over Mr. Proia's face, person, and vehicle and he was continuing to throw up blood. Mr. Proia was transported to St. Elizabeths Hospital in Boston by Newton Medics prior to my arrival. Due to Mr. Proia's condition at the time of his transport, Sgt. Michael Wade of the Newton Police Traffic Bureau contacted Trooper Mark Delaney of the Middlesex County CPAC who declined jurisdiction in the event that Mr. Proia succumbs to his injuries. I contacted Trooper John Paul from the MSP Reconstruction Unit to make him aware of the crash. MSP declined to respond. I notified Mr. Proia's daughter, Jennifer Proia, that her father had been transported to the hospital after being involved in the crash. Photos were taken of the crash scene and submitted to the IT Bureau. I observed damage to the front passenger side bumper/fender area of Mr. Proia's vehicle. I observed very minor damage to the front driver side tire area of the Volvo rock truck involved in the crash. Mr. Proia's vehicle was removed from the roadway by Tody's Towing. Officer Wilson canvassed the area for surveillance footage of the incident with a negative result. Sgt. Wade received a call from Dr. Grivetti (2021-10751) from the Medical Examiners Office a short time after Mr. Proia was transported to St. Elizabeths. Dr. Grivetti stated he received a call from the hospital notifying him that Mr. Proia has passed away and he was making notification to us. Dr. Grivetti stated it is his understanding that Mr. Proia suffered an aggressive GI bleed leading to cardiac arrest. This medical event appears to have been the catalyst for Mr. Proia's crash.

On 03/08/22 at 14:57 hours, N494 and I Traffic Unit # 523 responded to the Horace Mann School located at 225 Nevada St. for a two car motor vehicle accident involving one of the operator's being struck. The accident occurred on Nevada St. just past a driveway of the Horace Mann School approaching the intersection of Nevada St and Linwood Ave. The weather at the time was sunny/clear and the road conditions were dry. School dismissal is at 14:50 hours and for approximately ten minutes Nevada Street is very congested with vehicles parked on both sides of the street. On arrival, I spoke to the operator of M/V # 2 identified as James Rice (MA D/L # S83251000). Mr. Rice's vehicle is a 2012 Ford Escape color white bearing MA reg. # 6725ZA. The Ford Escape was parked just past a school driveway facing southbound in a legal parking spot. He says he was picking up his child from the school. He further states that he looked and thought it was clear to open his door. He then opened his door and put his left foot and left arm out attempting to exit the vehicle when it was struck by M/V # 1. M/V # 1 struck the outer edge of the door and Mr. Rice's left elbow was also struck during the collision. Mr. Rice complained of left arm pain so Officer Boudreau had Fallon Ambulance respond to the scene. Rice then signed a patient refusal with them. Also inside the vehicle at the time of the crash was his son, Charger Rice. His son was seated and secured in a car seat in the second row of the driver's side of the Ford Escape. There was no injuries reported for him. I observed a crease to the outside edge of his driver's side door. Mr. Rice also stated to me that the crossing guard told him the other operator was speeding before the crash going approximately 25 miles per hour. Next, I spoke to the operator of M/V # 1 identified as Olga Kulakova (MA D/L# S63003743). She was operating a 2015 Infiniti QX60 color black bearing MA reg. # 5TJ151 at the time. Ms. Kulakova states that she was parked approximately twenty five feet back from the crash location also picking up her daughter from the school. After doing the pickup, she then proceeded out in to roadway going southbound on Nevada St. As she was approaching M/V # 2, she says the operator suddenly opened their door and she was not able to react in time striking his car door. She did not believe she ever struck the operator though. No injuries were reported. I observed white paint scrape marks to both her passenger side doors and the side view mirror was also damaged. I then spoke to the crossing guard assigned to the intersection of Nevada St. and Linwood Ave. The guard Jack Boudreau is employed through the City of Newton. I asked him what had occurred with the crash. He stated that he had his back turned away from the crash location when it occurred. He heard the collision which made him turn around. He then motioned for the driver of the Infiniti to pull over and stop. I asked him if he ever saw the Infiniti driving prior to the crash and he stated that he did not. I then inquired about the supposed speeding comment that he made to Mr. Rice regarding the Infiniti. He stated that he never said that to him. Digital photos were taken of the scene and submitted to the I.T. Bureau.

Operator of vehicle one James Cellucci stated that he was driving south bound on Lexington St near number 419 in his 2012 Jeep Grand Cherokee ( MA reg 9339AC ). Cellucci stated his speed was approximately 10 to 15 MPH. Cellucci stated that a group of approximately 5 children ran out from between vehicles that where stopped in traffic in the north bound lane of Lexington St. Cellucci stated that one of the children ran in front of his vehicle. Cellucci stated that he attempted to break but a crash was inevitable. Witness Molly Van Horn made the same statement as Cellucci about a group of children running out between stopped vehicles . The child that was struck by Cellucci's vehicle was Ben Shreve ( DOB 12/14/2010 ) . Shreve did not lose consciousness as a result of the crash. Shreve was transported to Boston Children's Hospital via EMS for treatment along with his parents. Lexington St is a public way in the City of Newton.

On 03/20/222, I responded to Washington Street @ Hovey St for a 2 car MV accident. Upon arrival I spoke with the operator of MV 1, who stated that she was on the left lane on Washington St waiting to turn left onto Hovey street. She stated that the oncoming vehicles stopped for her to turn left on Hovey St when MV2 swerved around the vehicles and crashed into her causing major damage to the center front and right side of the MV. The operator of MV1 stated that she had a dash cam that caught the whole accident on video and they she would provide the video so I can view it. I then spoke to the operator of MV2, who stated that he was driving in far right lane Westbound on Washington St when the operator of MV1 turned left and hit him causing major damage to the center front and left side of the vehicle. He stated that he had the right of way and that there were no cars in front him which is why he continued driving straight. He also stated that he did nothing wrong and that he was following the speed limit. The operator of MV1 was able to send the dash cam footage from the accident. Based on my observations the video was consistent with the statement that operator of MV1 gave. In the video it is clear that MV1 had the right of way to make the left turn onto Hovey St. The operator of MV2 was mailed Mass Uniform Citation T1445888 fir M.G.L 89 4A Marked Lanes violation. I am mailing the citation due to the fact the that operator of MV2 had left the scene already. Medics arrived on scene and a patient refusal was signed. Todys arrived on scene and took possession of both motor vehicles.

On 3/28/22 at approximately 0825 hours I was dispatched to 27 Oak Street for a report of a minor motor vehicle crash that just occurred. The original call for the incident was placed by the operator of motor vehicle #1, LEVIN Nina. LEVIN stated to Dispatcher Ciccone she was just involved in a minor crash with her neighbor at 23 Oak St. NEVIN stated she was leaving to pick up someone that needed a ride but would be right back. She further stated there is a long standing and ongoing neighbor problem with the person in the other vehicle and she did not want to speak with her without an Officer present. Upon arrival at the address LEVIN was already back on scene and began telling me what had occurred when the operator of the other vehicle, DELORIE Diane, came down the driveway and became confrontational with LEVIN and myself. I Instructed her to go back to her vehicle and wait for me to come up to her. LEVIN continued to tell me that she was backing down the driveway toward Oak St. and as she backed onto Oak St. she did not see the other vehicle and struck the front of DELORIE's vehicle with the rear of hers. After speaking with LEVIN I went to speak with DELORIE and stated there was no need to be confrontational as there was no intent and this was simply an accident. I asked DELORIE what happened and she stated she was travelling Southbound on Oak Street and slowed to a stop to take a right into her driveway at 23 Oak Street. DELORIE stated LEVIN was backing down the driveway at a high rate of speed and backed out of the driveway striking the front of her vehicle. Both LEVIN and DELORIE have a shared driveway and DELORIE needed to wait for LEVIN to exit before she could proceed into her driveway. DELORIE stated to me she felt it was done on purpose and wanted LEVIN cited and charged for leaving the scene. I informed DELORIE this did not constitute Leaving the Scene of a Motor Vehicle Crash - Property Damage as LEVIN reported the crash before DELORIE called. DELORIE and her husband disagreed with my assessment of the incident. DELORIE's husband insisted this constituted Leaving the Scene and DELORIE insisted backing out of a driveway was not permitted by State Law. I informed both parties that LEVIN would be given a verbal warning regarding failure to use care in backing but no citations would be issued There was minor damage caused to the passenger side front bumper of DELORIE's vehicle and no damage to the rear of LEVIN's vehicle.

On Friday April 8, 2022 at approximately 1230 hours I was dispatched to 36 Glen Ave for a single car MVA into a stone wall. The sky was clear at this time of day but the roads were wet due to a previous rain storm. Witness stated MV1 was operating southbound on Glen Ave (public way) prior to 36 Glen Ave at approximately 40mph. Be advised the speed limit of Glen Ave is 25mph. Witness said MV1 crossed into the northbound lane, and operated through the driveway entrance and into the stone wall belonging to Glen Ave Association. Witness stated there were no other MVs on the road at this time and they were unsure if MV1 operator was on their cellphone while driving. Witness stated MV1 operator crawled out from the vehicle and there were no one else involved or in the car. MV1 sustained heavy and extensive front end damage, and was towed by Todys. MV1 operator stated they only had chest pains, and was not suffering from any head or neck injury at this time. Due to the nature of the incident and quick response by Newton Medics, I was unable to speak to MV1 operator, who was transported to Newton-Wellesley Hospital. I was able to speak to an employee of Glen Ave Association and advised them of the incident. The employee was the manager and they stated the damage to their stone wall was approximately $5,000 USD, and their telephone extension was x200. The employee was given the accident report number.

On 4/21/22, I was dispatched to the intersection of Crafts St./California St. for a motor vehicle collision involving a single vehicle into a tree. Upon arrival, I observed MV#1 had crashed into a tree in front of 315 Craft St. The operator stated the "road was slippery" and he veered off the roadway, hitting the curb, before traveling 20-25 yards on the sidewalk, hitting two tree's. MV#1 sustained heavy front end damage and air bag deployment consistent with colliding into a tree at a decent rate of speed. MV#1 was removed from the roadway by Tody's Tow Service. The operator of Mv#1 was transported to Newton-Wellesley Hospital with injuries. Charges pending at this time for OUI Liquor 2nd Offense, Marked Lanes Violation and Negligent Operation of a Motor Vehicle.

Owner of MV1 said she was standing in front of her house, 135 Randlett Park when she heard a loud bang. Owner of MV1 saw a landscaping company truck and trailer speeding away from her car. Owner of MV1 noted that there was a vehicle parked directly across the street making the street narrow but said trucks have been going up and down the street all day without any issues. Owner of MV1 said she could not make out the lettering on the truck but got the license plate which is 459567. Owner of MV1 said she's certain that this was the license plate but there is a chance she missed a number. I ran the license plate and it comes back cancelled to a 1988 American Command Pick Up truck and there is no contact information for the owner of the vehicle. It shows that the registration has been expired since 12/31/1989. Owner of MV1 showed me the damage that was done to the vehicle which was a flat tire on the rear direr side. Owner of MV1 made arrangements for AAA to come and take care of the vehicle. I provided Owner of MV1 with this accident report number. I canvassed the area for the landscaping truck with negative results. Owner of MV1 contacted me stating her neighbor has a picture of the truck off of their front door camera. Owner of MV1 emailed me the picture and I forwarded the picture to IT to be attached to this report. It should be noted you can not make out the lettering of the company in the photo.

At approximately 1840HRs on Wednesday, June 08, 2022, I was dispatched to 125 Parker St. for a road rage incident. Upon arrival, I spoke with Derek Eggleton, who stated that he was walking from his truck on Glenwood Ave. to 125 Parker St. with building supplies in hand. As he crossed Parker St., he noticed a Maserati bearing MA Plate: SUGAR approaching him, southbound, at a high rate of speed. Derek then threw said building supplies and moved out of the way of the vehicle because he believed it was not going to stop. He then approached the vehicle which had slowed down and Derek states it "tapped" him in the leg. Heated words were exchanged between Derek and the operator of the vehicle, identified as Sidney Gersh, who ultimately drove off. While I was on scene, Officer Hatfield spoke with Sidney, who stated we was indeed involved in the incident, but states he did not hit Derek. Officer Hatfield observed no damage to the vehicle and I observed no marks on Derek, who signed a patient refusal. I advised Derek that I would be documenting this incident and explained to him that if he so chooses, he may seek charges through Newton District Court. He stated that he understood and would take that into consideration.

Operator of Vehicle 1 states he was travelling Northbound on Ferncroft Rd when the operator became briefly distracted by his phone. Vehicle 1 then struck the curb and a tree on the West side of the road in front of #64 Ferncroft Rd, causing extensive damage to the front of the vehicle and airbag deployment. The operator denied any injuries and declined medical treatment. Vehicle 1 was towed from the scene by Tody's Towing. Newton Fire responded to clean up engine fluids that had leaked on the roadway. Mv#1 operator stated he was travelling on Elliot St E/B (right of way) passing the exit driveway of Negoshian's service station, 14 Elliot St. #1 stated at that time, a black Lexus SUV bearing MA reg H2158 (Mv#2) nosed out from the driveway and made contact with his passenger side rear quarter panel behind the wheel. #1 stated he pulled over on Elliot St however #2 did not stop and left the area. #1 described the operator as a W/M with dark beard and a female passenger. I observed #1 passenger side rear quarter panel which appeared to show fresh light colored scuff marks and light paint transfer. I queried the reported plate number of #2 which came back registered to Beverly Newman 16 Pickwick Rd W. Newton on a black Lexus SUV. Ofc Chieu responded to this address and located #2 there. The male operator was raised who identified himself as Peter Feig of the same address. The owner Ms Newman was the front seat passenger. I was able to observe a photo of the left front end of #2 and observed a light scuff mark where the grill meets the fender. I spoke to Mr Feig who stated he was unaware there was a collision at all. It appears the silver colored front grill of #2 made contact with #1. He stated he did remember #1 drive by at a high rate of speed. Feig was advised a report would be on file. #1 owner Mr Todd Miller was advised of my findings, satisfied and will contact his insurance company if needed.
