On 01/04/19 I responded outside of #54 Needham St for a two vehicle crash. Veh #1 was a delivery box van who was driving due North on Needham St. The operator of veh #1 stated he was attempting to take a left into #55 Needham St. when veh #2 came from behind him on his left and he struck veh #2 as he turned. The operator of veh # 1 was taken by Ambulance #1 to Newton-Wellesley Hospital for an evaluation after complaining of pain in his chest area. The passenger of veh #1 stated everything happened "so fast" he didn't see what transpired. Moderate damage to veh #1. The operator of veh #2 stated he was traveling due North on Needham St. and was behind veh #1 for a long stretch of the road. The operator of veh # 2 stated veh #1 put his right directional on three different times but didn't turn. He stated veh #1 had his right directional on and was in the right lane when he went to pass him. Veh #1 then turned into his rear passenger side as he passed. Moderate damage to veh # 2. The operator of veh #2 stated the operator of veh #1 was walking around and yelling about the accident for a period of time and then eventually grabbed his chest and fell to the ground complaining of pain. From the result of my investigation it appears as if veh # 1 attempted to turn from the wrong lane and veh # 2 was passing in the "suicide lane", also wrong lane. There will be no citations issued at this time as both operators were given verbal warnings.

On Wednesday, January 16th 2019, at approximately 3:30pm, I, Officer Brooks, responded to a single car MVA in the area of 611 Watertown street. Upon my arrival Cataldo medics were treating the operator of MV1(MA REG 159EF6) for a diabetic issue. I spoke with a witness to the accident who did not wish to leave their information. The witness stated they were traveling westbound on Watertown street behind MV1, when it suddenly left the roadway to the right, striking the curb, before returning to the roadway and stopping. The witness stated that MV1 nearly missed a fire hydrant and a utility pole on the sidewalk. MV1 had a flat front passenger side tire, and minor scrapes on the body. I spoke with the medics who were still treating the operator for her diabetic issue. I observed the operator was in and out of consciousness, and unable to speak about the accident. It took several minutes for the operator to regain her consciousness, however she was still not able to answer simple questions coherently and was transported to Newton Wellesley hospital for further evaluation. MV1 was towed from the scene by Tody's, and an inventory form was filed and submitted. The area the crash occured was a high pedestrian traffic area, and both an elementary and middle school are within walking distance of the accident. An immediate threat form was filled out and filed for the operator of MV1.

On the above date and time I responded to the area of St.James St at Washington Street for a report of a past hit/run with City traffic signal knocked over. Upon arrival I observed the the traffic signal on St. James Street westbound just before the Washington Street bridge on the roadway. I did observe what appeared to be a sidewalk plow tire impressions on the sidewalk to the traffic signal. I cnanvassed the area with negative results and I am waiting to hear back from Public works who may have been assigned the area to plow the side walks. I also checked for any cameras in the area with negative results. I took pictures of the crash site area and I will have them downloaded at th I.T. Bureau.

On 1/14/2019 at approx 1406Hrs while assigned to 497 I responded to the area of 2150 Washington St for a report of a 2 car collision. Upon arrival I met with the operators of Ma Reg 5NG581 Jeff LONDON and Ma Reg RT35K2 Lori GANS. LONDON was signing a refusal with Cataldo Ambulance and stated he had already exchanged all his information with the other operator. I spoke with GANS who stated LONDON stopped suddenly for a turkey that walked into the roadway, GANS was able to brake but lightly tapped LONDON.

Leary was crossing Needham St southbound at the corner of the property between Stop and Shop and Jiffy Lube. She states that she was 1/4 of the way across the street when she was struck by MV1. The left side of her body(arm, hip) struck the right side of the MV. She fell to the ground. MV1 states that he was pulling out of the Stop and Shop driveway to turn right onto Needham St. He saw the pedestrian next to the snow bank on the right side of the driveway as he approached the end of the driveway. When he got to the end he did not see her anymore and assumed she had continued walking west on Needham St. He then turned right and felt the collision and then he saw her. He immediately pulled into the Jiffy Lube lot to check on her. I took pictures of the scene and the MV. They were TOT IT. After further investigation, on 2/7 the operator was cited for 90/14 failure to stop for pedestrian. I spoke with Kenneth via phone and advised him of the update. The citation was mailed to him. I am charging for 90/14 failure to stop for a pedestrian. On Wednesday February 6, the operator information of this report was ammended to show the correct operator information.

On 01/31/19 at 10:39 hours, I responded to the parking lot of the Jewish Community Center located at 333 Nahanton Street for a motor vehicle hit and run accident. The parking lot is accessible to the public and designed for vehicles to travel to and from the JCC. Owner of M/V 1 states she was parked on the side of the handicapped parking stall thinking it was ok to park there. She then ran in to the JCC for approximately one minute and when she returned she observed her driver's side front door was struck. I observed significant damage to the door frame and the door would not close. The owner called for a private tow to remove the vehicle. The staff at the JCC stated that a Martha Bixby gave her number and stated that she observed a black colored car with MA reg. # 934HH4 strike M/V 1. I attempted to call the number and left a message. The listing for that plate comes back to a 2010 Lexus ES350 color gray. I was then showed the video surveillance from the JCC's cameras and it shows an older white female exiting the lobby wearing a winter hat and carrying a bag with purple flower designs on it. She then walks through the lot and enters a gray Lexus ES350. The Lexus then starts backing out of the spot. The Lexus then continues backwards in to the other side of the parking stalls and strikes M/V 1 on the driver's side front door. The Lexus then pulls forward and leaves the scene without stopping or providing her information. As the Lexus is leaving, you can see there is a dent to the driver's side rear bumper. The video is not clear enough to see the plate on the Lexus or clearly see the operator's face. After, I located a number for the owner of the Lexus M/V 2 (Tania Lefman). I spoke to Lefman on the phone and she stated that she was backing out of the JCC parking lot at that time and felt a tiny bump to her rear end. She says that she could not stop in time due to the icy parking lot and then left the area thinking that nothing was wrong, it was only minor. I advised her there was significant damage to the other car. She was issued and mailed MA Uniform Citation T1441054 for Ch. 90/24/C Leaving the scene of property damage.

OWNER STATES HIS VEHICLE WAS PARKED ALL DAY BECAUSE HE IS DOING WORK AT THAT LOCATION. COWORKER GOT THE PLATE OF THE VEHICLE THAT HIT HIS VEHICLE. OWNER REPORTS NO INJURY AND WAS ADVISED O CONTACT HIS INSURANCE COMPANY. ATTEMPTS TO MAKE CONTACT WITH THE OWNER OF VEHICLE #2 WAS UNSUCCESSFUL AT THIS TIME. FOUND A SECOND NUMBER ( 860-301-1336 ) FOR THE METROWEST PACKAGING COMPANY AND LEFT ANOTHER VOICE MAIL.WILL TRY TO MAKE CONTACT AGAIN BEFORE PURSUING OTHER OPTIONS. Traffic Bureau Update (Officer Gaudet): On Monday, February 4, 2019 I made contact with the President of Metrowest Packaging, Brian Hersh. Hersh stated his company does fulfillment deliveries for Amazon. Hersh was advised that a vehicle registered to his company was allegedly involved in a hit and run accident. Hersh was provided information on the vehicle involved. Hersh stated an employee named Alexis was operating MA CON:T65665 on the date of the accident and provided a phone number for this party. On Tuesday, February 5, 2019 I spoke with Alexis Laporte-Soto (S84358654). Laporte-Soto stated he was operating a 2017 Mercedes Sprint van (MA CON:T65665) in Newton for Metrowest Packaging on the date of the accident. I explained to Laporte-Soto the vehicle he was operating was alleged to have been involved in a hit and run accident on Highland Street in Newton. Highland Street is a public way maintained by the City of Newton. Laporte-Soto stated the vehicle he was operating hit the driver door mirror of a parked card on Highland Street. Laporte-Soto reported the accident as mirror (MV2 passenger side) to mirror (MV1 driver side). Laporte-Soto stated after his passenger side door mirror made contact with MV1's driver side door mirror, he stopped his vehicle 15 feet down the road. Laporte-Soto stated he walked up to the car he hit and saw the mirror was bent back but there was no damage. Laporte-Soto stated he waited near the car for 10 minutes and the owner never showed up. Laporte-Soto also stated a "random guy" took a picture of his license plate. Laporte-Soto stated he said "hi" to this party but the party ignored him and left the area. I asked Laporte-Soto why he did not call Newton Police to assist him in finding the owner of the vehicle he hit. Laporte-Soto stated he didn't know. I also advised Laporte-Soto that the name of the owner of the vehicle he hit was printed on the driver side door next. The owner's phone number and email address were also printed on the driver door. Laporte-Soto stated he didn't see that. I spoke with the owner of MV1, Jason Correria. Correria stated a co-worker observed the entire accident and observed MV2's passenger side mirror hit MV1's driver side mirror. Correria stated his co-worker then observed MV2 travel down Highland Street with out stopping and pull into the Santander parking lot located at 1314 Washington Street. Correria stated his co-worker saw the operator of MV2 exit his vehicle, check his passenger side door mirror for damage, and leave the area. Laporte-Soto will be mailled Massachusetts Uniform Citation T1443121 for Chapter 90, Section 24 (Leaving the scene of property damage).

On 02/08/2019, I, Officer Conary, responded to 309 Tremont Street for a past hit and run involving a pedestrian. Upon arrival, I was met by FEBLOWITZ, Colleen, who stated at approximately 1745 hours, she was crossing in the crosswalk at Tremont at Hibbard when her foot was hit by a car. FEBLOWITZ explained to me that she was in the crosswalk and she saw a silver sedan, MV1, traveling Eastbound on Tremont Street approaching her but it was far away where she felt safe to walk across the street. She said she also raised her hand to draw attention to herself. FEBLOWITZ was halfway in the crosswalk when she felt severe pain in her right foot. She looked behind her and saw MV1 was very close to her. MV1 continued to drive down Tremont Street and not stop. There were no witnesses. FEBLOWITZ went to Partner's Urgent Care to be treated for minor injuries. The crosswalk is in a lighted roadway. FEBLOWITZ was unable to gain any other information on MV1. No further incident to report.

MV#1 stated while traveling westbound on Commonwealth Ave he closed his eyes for a few moments causing his vehicle to veer to the right. MV#1 drove up on the curb and struck a overhanging street light. MV#1 sustained heavy front end damage as well as uprooting the overhanging street light. Operator of MV#1 stated he had minor injuries and signed a patient refusal. Operator of MV#1 was given the following roadside tests, due to the time and circumstances of the incident. Horizontal Gaze Nystagmus - Pass One Leg Stand - Pass Walk And Turn - Pass Operator of MV#1 showed no signs of impairment. Through my training and experience I calculated MV#1 had speeds of approximately +50MPH. Tody's towed the MV, a towed MV inventory form was completed. Pictures taken on scene submitted to IT.

On my arrival I spoke with the operator of MV1(MA REG 28DG82), who stated his vehicle was struck at 3pm this afternoon in front of 1296 Washington street. His vehicle was alledgedly struck by a bus, unknown plate or company, that was traveling eastbound on Washington street. The bus casued the front drivers side quarter panel to be ripped from the vehicle. MV1 was towed by AAA.

The operator of Vehicle #1 returned to her vehicle to a note from a witness that stated that she witnessed a vehicle strike her front bumper, as it was reversing to park. I also observed a small dent and a scratch consistent with a collision of this type on Vehicle #1's bumper. All attempts to contact Vehicle #2 have been negative, of yet. I will follow up on my next tour. The witness, identified as Ms. Michelle Temple, stated that she observed the collision, and added that it was with enough force to cause damage to the bumper. She then stated to the operator, "you just hit that car", to which the operator replied, "I know", and then walked away towards Newtonville Square. I was later able to speak with Ms. Rogovin. She stated that she didn't believe any damage was caused, thus she didn't notify anyone.

Vehicle 1 was parked unoccupied in the employee parking lot of Newton Wellesley Hospital when it was hit by Vehicle 2. The operator of vehicle 2 stated she was correcting her parking job and tried to back up, but the car was actually in drive and not reverse. the car drove forward hitting vehicle 1 going over a curb and hit a tree a few feet past the curb. This caused minor damage to Vehicle 1 and major front end damage to Vehicle 2 as well as front air bag deployment. Since this occurred on hospital grounds the operator of vehicle 2 walked herself to the ER for precautionary reasons and was cleared. Vehicle 2 remained in the parking area for a private tow company.

Narrative:
I responded to the area 107 Dartmouth St. at approx. 0345 on 04/28/9019 along with N493 Officer Kelly Donahue and N508 Sgt. Devine for a report of an MVA. The MVA was reported to be single vehicle crash. The vehicle left the roadway and was on the front lawn of 107 Dartmouth St. I arrived on location (first on scene ). I observed tire tread marks across the front lawn of 107 Dartmouth St. I then observed a vehicle in the bushes toward the left front side of the house. The vehicle; NY Reg. JHE8252, Porsche color blue. The RMV info on the plate came back registered to Porsche;of ;South. Registration plate type: Dealer Assignment. 185 W Sunrise HWY; Freeport,NY (no further info). The vehicle was unoccupied and secured. I felt the hood of the car which was still warm from the engine. I checked the immediate area surrounding the crash scene for an operator but found no one. I radioed to other responding units to canvas the neighborhood and surrounding area for the operator on foot. I spoke to the homeowners at 107 Dartmouth St., identified as Anthony and June Limberis. Mrs. Limberis stated she heard a load noise and a car engine revving. She stated when she looked out the window she observed a W/M tall, thin, wearing tan khakis and a light colored plaid shirt walking across the front lawn. He proceeded up Dartmouth St. (northbound) toward Commonwealth Ave. Newton Fire Dept arrived on location to check the crash scene and vehicle for possible leaking fluids and any other hazards. As I was talking with the firefighters we were approached by two men. One of the men was intoxicated. He was unsteady on his feet, had glassy bloodshot eyes, and was slurring his speech. The man was a W/M tall/thin approx. 6'2" and was wearing tan khakis and a light colored plaid shirt. He was identified as Eric Adams DOB 09/18/1969. The second man with him was identified as John Adams DOB 02/07/1936; An elderly W/M approx. 5'6" also wearing tan khakis and a light colored plaid shirt. Eric Adams stated his father John was the operator of the vehicle. Eric Adams had a strong odor of Alcoholic beverage emanating from his person and breath. He continued to slur his speech and was unsteady on his feet as he spoke. He was swaying back and forth and side to side as he spoke to me. Officer K. Donahue and Sgt. Devine were on scene at this time and also made the same observations. Multiple times we had to grab hold of Eric Adams to keep him from losing balance and falling. We spoke with Eric and John Adams for several minutes as their accounts of what took place were not consistent. I asked Eric Adams where the keys to the vehicle were. Eric Adams immediately pulled the starter key to the vehicle from his front right pocket. Eric Adams became defensive and belligerent when questioned about who was operating the vehicle. John Adams appeared tired (as if he just woke up) and confused as to the situation. Sgt Devine and I both spoke with Eric Adams and John Adams. Although both men claim John Adams was the operator of the vehicle, a clear determination was uncertain. A query of both mens driver's license status on NY Licenses came back active. Eric Adams has two prior DUI's on his RMV record. Officer Donahue and Sgt. Devine escorted the two men to 93 Dartmouth St. Eric Adams stated they were staying at that residence which is his sister's house. I stayed at the crash scene to document information and assist with clearing the MV from the property with Tody's Towing Service. As they were walking away I observed Officer Donahue and Sgt. Devine continue to have to assist Eric Adams with staying on his feet. Approximately ten minutes later as I was speaking with the aforementioned homeowners Mr.&Mrs. Limberis. Officer Donahue and Sgt. Devine returned to the crash scene. As they were updating me we observed Eric Adams returning to the crash scene. He was stumbling and swaying as he walked and repeatedly almost fell. Officer Donahue and Sgt. Devine immediately intercepted and assisted him before he fell. As they were assisting him again I continued speaking with the homeowners. Suddenly I heard Officer Donahue and Sgt. Devine struggling with Eric Adams who was now on the ground and resisting being handcuffed. I ran to assist them. Eric Adams continued to resist and be uncooperative until he was appropriately restrained in handcuffs. Eric Adams was placed under arrest by officer Donahue for Disorderly Conduct and Resisting Arrest. Reference Officer Donahues Newton Police Incident Report #19016484. The vehicle NY Reg. JHE8252 was removed from the property of 107 Dartmouth St. and towed from the crash scene by Tody's Towing Service.

Narrative:
MV1 was traveling east on Madison Ave from Walnut St approaching #80. There were 2 landscape vehicles parked on left side of the street. The operator, Sarah Ruth Loper-Sengupta, stated that she never saw the pedestrian that was in the roadway (N) due to solar glare. Loper-Sengupta stated she heard a thump coming from the passenger side of her vehicle. Loper-Sengupta stated she looked over to her right and saw a trash barrel in the air. Loper-Sengupta stated she then saw a male party lying on the roadway on her passenger side. Immediately after striking the pedestrian, Loper-Sengupta, stopped to check on him. There was a golden doodle dog in the back seat of the MV at the time of the accident, unrestrained. There were no obstructions hanging from the mirror or windshield area. The front drivers visor was down. There was no airbag deployment. I observed significat damage to the front passenger side windshield area. There was also damage to the front passenger side hood area. See Sgt. Lee update for victim statement Numerous pictures were taken by Ofc. Gaudet of the MV and scene. They were TOT IT. The MV was towed by Tody's due to its shattered windshield. No citation will be issued to Loper-Sengupta. The solar glare caused her to not able to see the pedestrian. The Sgt. and I dont believe the pedestrian looked before crossing with the barell on his left shoulder because he would have been able to see the MV coming as this is straight away section of Madison Ave. It should be noted, that when I arrived on scene I immediately noticed the seolar glare as I walked east on Madison Ave.

Narrative:
On Tuesday May 7th, 2019 at approximately 13:15 hours I responded to the area of 30 Langley Road for an auto vs. pedestrian accident. At the time of the accident the weather was sunny and the road conditions were dry. Langley Road is a public way maintained by the City of Newton. Upon arrival I first spoke to the pedestrian who was inside the ambulance getting medical attention. The pedestrian identified as Donna Kalikow (S24151638) stated she was standing in an open parking space for a few seconds, attempting to walk across the street to enter J.P. Licks. Donna stated she was standing in the marked parking space waiting for traffic to stop to safely cross the street. Donna states the cars stopped to let her cross and she proceeded into the roadway. She states MV1 was attempting to back into the parking space that she was standing in and struck her right side and knocked her to the ground. Donna states the operator of MV1 stopped the vehicle when he realized he had struck her and got out to contact 911. Donna appeared to have minor injuries and complained of head pain. Medics transported Donna to NWH to be further evaluated. The operator of MV1 identified as Christopher Moos (S24090966) stated he was attempting to parallel park into an empty parking space and never saw the pedestrian standing in the space. Christopher stated as he was backing up into the space he was notified by a bystander that he just struck a pedestrian. Christopher stated he immediately stopped the vehicle and got out to assist the pedestrian and contact 911. Christopher stated he was not travelling no more than 5mph at the time of the accident. MV1 did not have any damage to the vehicle and was able to leave the scene. Christopher reported no injuries. Several photographs were taken of the scene and submitted to the IT Bureau. I mailed the operator a Massachussetts Uniform Citation for a City of Newton Ordinance violation for Section 19-75 Failure to use care when backing.

On 05/10/19 I met with Luiz A. Lopes who stated his vehicle ( Mass Commercial Reg# T83695 White Toyota Tacoma P/ U) was struck while parked in the lot around 07:00. Lopes said he didn't call the Police right away because he wasn't sure his car was struck there. The truck sustained a cracked front bumper and the fog light was popped out. We went inside of National Lumber and spoke with a manager who said he would contact G.M. Greg Assarian and have him look at the security footage. On 05/14/19 at approximately 14:00 the Police Station received a call from Lopes who just got off the phone with Assarian who found some security footage of the incident. I then traveled over to National Lumber and met with Assarian. He showed me the video and it appears that a newer grey four door Chevrolet truck backed into Lopes truck around 06:58 of 05/07/19. The Massachusetts plate is unreadable but an older White heavy set male gets out of the truck and walks into the store. After parking the truck the male can be seen checking his own truck bumper for damage. The male is wearing a baseball hat and sun glasses and cannot be identified on the video. The male does however walk with a noticeable limp. Assarian will show his picture to his employees and contact me if the employees are able to identify the male driver. I called Lopes back and apprised him of my findings. ******************UPDATE**************************************** As of today 05/20/19 I have yet to hear back from anyone at National Lumber so I will end this investigation and label it a hit and run crash.

On 5/20/19 at about 1510hrs I was dispatched to 136 Evelyn Rd for an MVA. En route to the location with Ofc Gourdeau, it was determined by first arriving Ofc Gourdeau that a vehicle had crashed through a fence at the end of Troy Lane and came to rest in the backyard of 126 Evelyn Rd. On arrival, I located the vehicle operator, identified as Mr Stuart Phillips who was operating MA Reg 8VVP40, an '03 Jaguar S Type model. Mr Phillips was walking around the backyard after the collision and appeared coherent as to what had transpired. He sustained a laceration to the top of his head and was bleeding heavily. We sat him down in a lawn chair prior to medics arrival to evaluate. When asked what happened, he stated he "hit the gas instead of the brakes". I asked him where did the accident begin? He stated he was turning around at the end of Troy Lane (a dead end cul de sac) and crashed through the fence there. Mr Phillips was alert and speaking clearly. I asked him if he was on any medication? He stated he was taking Plavix only, a blood thinner. I asked about his medical history and he stated he was released from Spalding Rehab today for treatment from a stroke and was receiving occupational therapy. Medics arrived to evaluate and he was transported to NWH for treatment. An investigation of the crash path of the vehicle revealed the following based on tire tracks and damage left behind: The vehicle crashed through the fence at the end of the cul de sac belonging to 41A Helene Rd; Vehicle drove over that back lawn, clipped the back deck attached to the rear of the house; damaging the base of the deck and lattice below; Vehicle travelled over the patio, then crashed through shrubbery down an embankment leading to the backyard of 132 Evelyn Rd; Vehicle drove through backyard of 132 Evelyn, then continued along the side of the house where it drove over damaging three retaining walls and clipped a large tree; After driving over the third retaining wall, vehicle continued through the backyard of 126 Evelyn, causing significant lawn damage; Vehicle continued until it hit the back of the house at 126 Evelyn; there, it drove over the rear patio before it struck a ground level basement window and window well; It came to a rest there where the vehicle caught on fire under the hood. Witnesses helped extinguish the fire with a garden house (see incident report #19020129) until NFD arrived to knock it down. Significant front lawn damage to 126 Evelyn occurred as well as NFD was arriving on scene to what was first reported as a structure fire. Several photos of the area were taken by myself. The memory chip was TOT the I.T. bureau to be attached to this report. Tody's towing towed the vehicle out of the backyard by accessing the rear driveway of 132 Evelyn Rd. Mr Phillips cited for Negligent Operation and an Immediate Threat for License Suspension/Revocation faxed to the RMV. Troy Lane is a public way in the City of Newton.

Tipruthai Thangthong stated that on 05/21/2019 at approximately 16:36 hours she was pulling into the driveway of her sisters home at 53 Rangeley Rd while operating her vehicle MA reg 2LVL61 a 2018 Honda Accord. Thangthong stated that her three old son Mighty Mettanant was a passenger in her vehicle. Thangthong stated that she was driving approximately 5 MPH as she drove into the driveway. The driveway runs east to west and Thangthong's vehicle was heading west into the driveway as the sun was in the Western sky creating a glare. Thangthong stated that she did no see anything in the driveway prior to her pulling into it. Thangthong stated that she felt the front of her vehicle strike what she described as " something " and stopped her vehicle. Thangthong stated that she put her vehicle in reverse and then into park and exited her vehicle to see what she had struck. Thangthong stated that she saw her two nieces, Kalla Mongkhonlertsirkul age three and Kelley Mongkhonlertsirkul age seven laying in the driveway in front of her vehicle. Thangthong stated that she then realized that her vehicle had stuck her two nieces as they were in the driveway. Both of the Mongkhonlertsirkul sisters were conscious and alert and were able to walk to an ambulance for transport to Newton Wellesley Hospital for treatment. Thangthong stated that she and her son were not injured in the crash. I observed no damage to Thangthong's vehicle. I responded to the Newton Wellesley Hospital and spoke with the Mongkhonlertsirkul sisters and their mother Piyada Kellum. Kellum stated that her oldest daughter Kelly is a student at the Franklin Elementary School . Kellum stated that her daughters told her that they were playing in their driveway when their aunt's vehicle entered the driveway and did not stop. I spoke with both of the Mongkhonlertsirkul sisters who told me that they tried to get out of the way of their aunt's vehicle but could not in time. The Mongkhonlertsirkul sisters stated that they were both struck in the back and fell to the ground between the two front tires of their aunt's vehicle. Kalla Mongkhonlertsirkul had scrapes to her right knuckles. Kelly Mongkhonlertsirkul had lacerations to her lower left arm and the left side of her face near her eye. I spoke with hospital staff who stated that there did not appear to be any broken bones or other internal injures. I took photos of the crash area and forwarded the disk to the NPD's IT bureau for downloading.

Operator of vehicle one, Benjamin Beaver stated that while driving south bound near 321 Walnut St his vehicle ( MA reg 551DK8 2014 Lexus ) made contact with a pedestrian in a marked crosswalk. Walnut St is a public way in the city of Newton. Beaver stated that he did not see the pedestrian prior to the crash and that his vehicle was travailing approximately 5 to 10 MPH. Beaver stated that the pedestrian was knocked to the ground but got up and walked around. The pedestrian, NNHS student Benjamin Donovan stated that he was crossing east to west in the marked crosswalk in front of 321 Walnut St. Donovan stated that he saw Beaver's vehicle approaching him at approximately 5 to 10 MPH. Believing that Beaver was stopping to let him cross , Donovan stated that he held up his left hand to signal to Beaver that he was going to cross. Donovan stated that Beaver's vehicle struck him on his left side and knocked him to the ground. Donovan stated that he had minor scrapes on his right side as a result of falling to the ground. Donovan stated that he did not lose consciousness and was transported via EMS to Newton Wellesley Hospital for treatment. I took photos of Beaver's vehicle and forward the disk to NPD IT Bureau. I issued Beaver MA Citation T 1269995 and cited him for a violation of MGL Ch 89 C 11, failure to yield to a pedestrian in a crosswalk. On 06/12/2019 I was forwarded an E-mail from Capt Anastasia of a video of the crash that was taken from a camera that was mounted on the roof of 321 Walnut St. The video shows Donovan walking into the marked crosswalk from the east side of Walnut St and hold up his left had to Beaver's on oncoming vehicle. Beaver vehicle appears to be travailing faster than the 5 to 10 MPH that Beaver believed that he was travailing. The video shows that Beaver did not brake until Donovan was struck and landed on his vehicles hood. Donovan was thrown approximately 10 feet from Beaver's vehicle and landed on his right side on the roadway. The video shows Beaver stopping and pulling to the side of the roadway after the crash.

Operator of Vehicle # 1 states that he was traveling straight ahead (NB) at 148 Parker Street when he stopped for approximately five children who signaled the cross walk signal to cross there. While completely stopped, he was rear ended by Vehicle # 2 and his vehicle was pushed forward approximately ten feet. His vehicle sustained major rear end damage and it was towed by Tody's. There is a functioning crosswalk signal there which has overhead lights that flash yellow before turning red. Operator of Vehicle # 2 states she was traveling straight ahead (NB) on Parker Street when she observed the above mentioned children on the side of the road. Next she says that she saw the crosswalk lights above flashing yellow but never saw Vehicle # 1 stopped there until the last second. She was then not able to stop in time striking # 1 in the rear end. Her vehicle sustained heavy front end damage and it was towed by Tody's. The operator complained of neck pain but signed a patient refusal with Cataldo.

On 06/28/2019, while assigned to West Newton, I, Officer Conary, met with Owner of MV1 for a report of a past hit and run accident. Owner of MV1 explained to me that at approximately 0800 hours they parked MV1 on Walker Street on the side of the road. At 1700 hours, they observed a dent on front right bumper. There was a note left that stated "a green van hit your car and scraped - License plate C49807". There was no additional information. The registration comes back to a party out of Somerville MA, Frank Sarno Jr. Somerville PD was contacted to check the residence. They left a note to have Sarno call Newton Police. Investigation is still ongoing until all parties involved have been contacted.

Operator of MV1 states she took Lake Ave westbound heading towards Hyde St when she was fighting off sleep and must have passed out at the wheel and struck a city tree on the eastbound side of Lake Ave in front of 160 Lake Ave. Operator of MV1 states she contacted AAA at 3:37am, but they never showed up so she abandoned the vehicle and walked home. Operator of MV1 states she didn't sustain any injuries and refused medical attention. MV1 sustained heavy damage to the left front fender and was towed by Tody's.

On 9/08/19 Kevin Coston came into NPD Headquarters to report a MV/Ped with Injury that had occurred on 8/23/19 at about 06:15 in the parking lot of Whole Foods (916 Walnut Street). The RP stated that he had left work (Whole Foods) at the back receiving door and was walking to his car, he bent over to take his knee pads off, while he was doing this he was struck by a delivery van that backed into him. The driver took out his headphones and said sorry before driving away. RP stated that he told the driver that he had been hit but the driver just drove away. RP went to hospital (Faulkner) on his own for lower back pain and has not been to work since. RP was able to take picture of the side of the delivery van. The van belonged to Shortstop Distribution Inc (508-655-5948). RP left a message that morning. He was texted by Gedeon (786-352-6695) who stated there was a language barrier and he didn't understand what had happened. RP advised Gedeon to talk to his lawyer or the police. Exact location of accident unknown. Diagram is a estimate. Report printed out and placed in traffic box for follow up. Traffic Bureau Update (Officer Gaudet): On Monday, September 9, 2019 I attempted to make contact with the operator if MV1, Gedeon Duarte, with a negative result. A Hit and Run motor vehicle inquiry was mailed to Duarte. I spoke with the injured party, Kevin Coston, via telephone on September 9, 2019. Coston stated he is an employee at the Whole Foods located at 916 Walnut Street, Newton. Coston stated on August 23, 2019 a black van backed into him as he was kneeling on the ground taking off his knee pads in the loading area of the store. Coston stated he approached the driver who remained in the vehicle to speak with him after he was hit. Coston described the driver as a heavy set Spanish male. Coston stated the operator of the vehicle rolled down his window, took his headphones out of his ear, said sorry, and left the area. Coston stated he contacted the Newton Police to report the accident but was told he needed to come in person to file the report. Coston stated he was unable to come to the station to file the report until September 8, 2019. On Friday, September 13, 2019, I spoke with Gedeon Duarte at the Newton Police Department. Duarte stated he spoke Portuguese and did not speak English. With the assistance of David Sargent from the Language Line (1-800-225-5254) translating, I was to speak with Mr. Duarte in the Traffic Bureau. Duarte stated he was making a delivery at Whole Foods and was waiting for another truck to leave so he could leave the area. Duarte stated a male party (Coston) came up to his driver door and was yelling him. Duarte stated that due to language barrier he was unable to understand what the person was saying to him, but stated the male party was yelling, "Fuck you! Fuck you!" Duarte stated he did not know what was going on and stated he left the area to allow the male party to calm down. Duarte stated he returned a minute later the male party did not want to speak with him. Duarte stated he returned a second time later in the morning and Coston was not there. Duarte stated he received a call from his employer stating Kevin Coston had called for him and he was provided with Coston's phone number. Duarte stated he texted Coston to find out what happened that morning. Duarte showed me a text message exchange from August 23, 2019 at 9:03 A.M. between himself and a party listed as "Kevin Whole". In the text messages, Duarte stated he is the driver of the van and that his supervisor called him. The other party responded and stated "Talk to the police and my lawyer". Duarte stated that he did not understand what happened and that he returned to whole foods. Duarte ended the text exchange with "927535425 my number of policy..." Duarte stated he had no idea he made contact with Coston and stated at the time of the alleged accident he did not hear/see/feel anything otherwise he would have stopped. Duarte's information has been updated in this report to reflect him as the operator/vehicle involved. Duarte also submitted pictures of the rear of his vehicle, a 2008 black Ford E350 (MA: 8NY171), from the day of the accident. Photos were submitted to the IT Bureau. Officer Wade went to the Whole Foods located at 916 Walnut Street, Newton in an attempt to view surveillance footage of the accident. Officer Wade stated the surveillance camera did not capture the incident and the van not viewed at all.

Rizza stated he waited for the crosswalk signal to turn on. When it finally turned on to walk (white walking person) he got on his bike to start peddling. At this time, he observed a black vehicle traveling southbound traveling towards him. Moments later, Rizza was struck by the vehicle. Rizza rolled over the vehicle and hit the pavement. Rizza stated the vehicle took off, but he was unsure where it went (Mass Pike, Newton Corner, or Washington Street). The medics looked at Rizza and he was alert and had a minor laceration to his head. Rizza's mother Domenica Rizza picked him up and brought him to NWH. I drove around the area and could not find a black Honda sedan with damage to the front of it. As of now, I do not have anymore information. This report is a hit and run. Traffic Update (Officer Gaudet): Myself and Officer Anderson have both canvassed the area of the crash for identifying physical evidence from the vehicle that fled the scene with a negative result. We have also reached out to local business's in an attempt to secure surveillance footage of the crash with a negative result. At this time we are unable to obtain any further information on the vehicle that left the scene of the crash.

Operator of MV1 stated he was driving southbound on Lake Ave and looked down for a brief moment and did not see the bend in the road and struck the stone wall in front of 219 Lake Ave. The passenger of MV1 stated the operator was traveling southbound and did not see the turn and went up on the side walk and collided with the stone wall. The operator of MV1 was evaluated by Medic 1, the passenger reports no injuries. Cataldo received two parental refusals for both parties. A witness (Baptiste, Franck Jean) stated he witnessed the accident and believed the vehicle was travelling at approximately 40mph and lost control around the bend and struck the wall. David Donahue from Tody's Towing towed the vehicle. The homeowner at 219 Lake Ave was home and notified about the damage to their property. Six digital photos of the accident scene were taken and submitted to IT. Update 9/28/19: Upon further review and discussion with Sgt. Dempsey, Operator of MV1- Gallivan, Chase was issued citation T1445308- 89/4A- Marked Lane Violation and 90/8-JOL Passenger Restriction. Citation was issued in hand.

On 09/24/2019 at 1649 hours, I responded to the area of 257 Pearl St for a two car MVA. Upon arrival I did not observe any damage to either vehicle. The passenger of MV1 was out of her vehicle yelling at the driver of MV2 and repeatedly walked back and forth between the vehicles confronting the driver of MV2. Both the driver of MV1 and MV2 stated that a vehicle backed out of a driveway in front of them. MV1 slowed down and MV2 rear ended MV1. The operator of MV1 states that she was traveling northbound on Needham Street with flow of traffic when she struck a pedestrian with the right front corner of her vehicle. The operator of MV1 stated that she did not see the pedestrian in the cross walk until after she hit her and that there was poor lighting in the area. Through the help of coworker from Anthony's Coal Fired Pizza, the victim stated that she was crossing Needham Street in the crosswalk and was almost on the side walk when she was struck by MV1 from behind. MV1 sustained minor front end damage while the victim sustained noticeable injuries and was transported to Newton Wellesley Hospital by Cataldo. Photographs of the scene were taken and submitted to IT to be attached to the report. The operator of MV1 was issued a Massachusetts Uniform Citation (T2079284) for 89-11, failure to yield to a pedestrian in a crosswalk.

He then heard what he believed to be a crash and observed his vehicle rock back and forth as Vehicle #1 backed into Vehicle #2. I observed no damage to Vehicle #2. Both vehicles were driven from the scene and no injuries were reported.

On 11/12/2019 at 12:26 hours, I responded to the Star Market parking lot located at 2040 Commonwealth Avenue for a motor vehicle pedestrian accident. The parking lot is a public/thru way that motorists use to access the Star Market along with other businesses. The weather at the time was cloudy and rainy and the parking lot was saturated. Prior to my arrival, the pedestrian involved left the scene and did not want to speak to the officer nor wanted medical attention. Ofc. Claflin then cancelled Cataldo Ambulance. The pedestrian, Mary Alexander, left her information with a store employee prior to leaving. I spoke to the operator of MV # 1 (Joshua Randel MD Lic.# R534440603244) who stated that he was travelling through the third row of the Star Market parking lot at an appropriate speed. He was then attempting to make a left turn near the entrance/exit of the store. He says that there was a large white van parked in the end handicapped spot towards the left of him. He says the van was sticking out in the spot limiting his visibility. As he was half way through his left turn and coming around the corner, he was not able to see a pedestrian pushing a shopping carriage. He then struck the carriage that was being pushed by Mary Alexander. He further says that the cart jolted Ms. Alexander and she fell down to the ground. At the time, Alexander was walking through the lot near the store entrance heading back towards the parked cars. She was not in the crosswalk at the time which was approximately ten feet down from her location. After, I attempted to go to Ms. Alexander's residence to follow up. I spoke to her friend who was bringing up Alexander's groceries and she stated that she decided to go to Newton Wellesley Hospital for treatment. I then spoke to Ms. Alexander in the lobby of the hospital. She stated that she was exiting the store through the lot with her shopping carriage. She believes she was approximately eight feet away from the door when all of a sudden she only saw red coming at her. She believes MV # 1 was speeding at the time. I asked her if she was struck by the vehicle or was the shopping carriage only struck. She replied that she did not know and only remembers falling to the ground after. She sustained a large bruise to the inside palm area of her right hand and her left knee was swollen. Digital photos of the scene were taken and pictures of Ms. Alexander's injuries were taken at the hospital. As far as possible video surveillance, I was told that the Loss Prevention Department has to access the video. On 11/13/19 at approximately 14:20 hours, I went in to the Star Market and spoke to the manager, Nidal Rajeh, inquiring about possible video surveillance. He said there is coverage in the parking lot and he was able to show me the video. A copy of the video would have to be done by Loss Prevention which I requested. I observed at the 12:12pm mark in the video, MV # 1 was parked five spots up from the third row of the lot that he was exiting. I also observed a white transport van parked in the end handicapped spot mentioned by the operator but it was parked correctly and not limiting visibility. Next, MV # 1 backs out of the spot and drives forward going at a reasonable speed. The pedestrian (Alexander) has already exited the store with her carriage and is walking at an angle toward the same third row of parked cars. As the pedestrian is walking towards the direction of MV # 1, it appears that the pedestrian should be visible to him. MV # 1 then starts to take the left turn around the corner without slowing or stopping and strikes the carriage head on. Due to the impact, Alexander then loses grasp of the carriage and it goes past her twisting her body around. She then hits and rolls up on the hood of the vehicle on her left side. MV # 1 then suddenly stops and immediately gets out and provides assistance to the pedestrian. Due to the video surveillance, Mr. Randel will be issued/mailed MA Uniform Citation # T2012347 for Ch. 90/14/A Failure to slow for a pedestrian.

MV#1 was traveling North on Fair Oaks Ave, and attempted to make a right turn onto California St. MV#2 was parked in front of 589 California St facing West. MV#1 appeared to have run over the curb at the intersection, hit a city tree in front of 55 Fair Oaks Ave (California St side), and then crashed into MV#2. When I arrived on scene I observed a male party with blood on his head, and another male party standing next to him, later identified as Elmer Lopez.. I asked Elmer which car he was driving, and he stated that he was in MV#1. I asked Elmer what happened, and he stated that he and the other man with him, identified as Ricardo Deras, were in the rear seat with another male party, and that there were two other parties in the front seat, one of them was driving, he was not able to provide me with a name of who was driving. I asked him where everyone else was, and Elmer stated that after the crash, the got out of the car and ran away. At this point, medics arrived on scene and began to tend to Ricardo. I next spoke with the owner of MV#2, Jim Silva. Jim stated that he was in his home when he heard a loud crash. Jim stated that he came out of the house in time to witness the two males I was speaking with looking at the car, and he also stated that he observed three parties walk away from the scene, two heading east on California St, and one heading north on Fair Oaks Ave. MV#1 and MV#2 were towed off scene by Todys. Due to the scene, and the location of the cars in a busy street, I was not able to do a MV inventory of MV#1 until after it was towed by Todys. The inventory was done at Todys Service station, and a MV inventory sheet was filled out and placed the in appropriate box. I took three pictures of the damaged tree and submitted them to IT to be attached to this report. While I was taking the pictures of the tree, two males were walking past the scene. I stopped both parties and questioned them about the incident. They stated that they had been in the car, but had not been driving. I asked them if the knew who the driver was, and they told me that they did not know the name of the driver. Due to no witnesses observing the driver and all parties stating that it was someone else, I was not able to determine a driver of the vehicle. Units checked the area for the fifth party in the vehicle with negative results. Ricardo was transported to NWH hospital by medics. All other parties were sent on their way. All units cleared without further incident. **UPDATE** On Nov 25, 2019 at approximately 1621 hrs, I contacted Westborough Police Department in an attempt to get a hold of Dora. Westborough informed me that they made an attempt to contact her with negative results. Westborough PD left my contact information for her to call me back.

Operator of Vehicle #1 was traveling on Saw Mill Brook Parkway before turning right over a curb and onto Kappius Path. Kappius Path is a walking path not intended for vehicle traffic. While traveling on Kappius Path the operator struck a tree with the driver's side front of the vehicle. The vehicle then rolled onto the passenger side. In order to extract both occupants of the vehicle, Newton Firefighters used the Jaws of Life to remove the roof of the vehicle. Firefighters also had to remove several sections of a chain link fence belonging to the property owner of 19 Kappius Path. Newton Medics obtained patient refusals from both occupants. No injuries were reported. The vehicle was towed from the scene by Todys and an inventory sheet was completed by Officer Emmanuel. Five photographs of the scene were submitted to the IT Bureau. The operator of Vehicle #1 was subsequently arrested and charged with OUI, Negligent Operation and Marked Lanes.

Operator of vehicle one, Christopher Sermak, stated that while turning north bound in the parking lot of 175 Boylston St he was blinded by solar glare. Sermak stated that the diver's side visor was down and the he was not wearing sun glasses. Sermak stated that he heard a loud bang coming from the front driver's side of his vehicle and realized that his vehicle had stuck something. Sermak stated that he stopped his vehicle and saw a male identified as John Baker laying on his back on the ground. Baker stated that he had step off the side walk in front of 175 Boylston St and was walking to his car when he saw Sermak's vehicle approaching him and realized it was not slowing, Baker sated that the front driver's side of Sermak's vehicle struck his right arm and shoulder causing him to be knocked to the ground on his back. Baker stated that his head struck the ground but he did not lose consciousness. Baker was transported to Newton Wellesley Hospital via EMS for treatment. While on scene I observed that the rear passenger side brake light of Sermak's vehicle was out. I issued Sermak MA Citation T 2012527 and cited him for violations of City of Newton City Ordnance 19-75, failure to use care while tuning , and MGL 90/7 , defective lights.

On 12/23/19, at 1919 hours, I responded to the area of 807 Washington St. for pedestrian struck by a motor vehicle. Upon arrival with Cataldo Medics and Newton Fire, I observed Catherine Chapman being treated in the westbound lane a few feet in front of a 2010 Hyundai Elantra (MA Reg. 4KH 743). The vehicle had a shattered windshield. Ms. Chapman appeared to be wearing a dark colored jacket and dark pants. The operator of the motor vehicle, Emily Shuman, stated she was traveling westbound in the outside lane when Ms. Chapman stepped in front of her car from the eastbound lane. I also spoke with a witness, Malcolm Robinson, who stated Ms. Chapman was crossing the street from the eastbound lanes with two other people. The other two people stopped in the middle of the street but Ms. Chapman kept walking and was struck. The other two people were Victor Polk and Halimah Polk, both of which confirmed the witness statement. Ms. Chapman was transported by Cataldo to Brigham & Womens Hospital with non-life threatening injuries. Todys towed the vehicle and pictures of the scene were submitted to IT.

Operator of vehicle 1 stated she was parked in a parking spot when vehicle 2 started pulling headfirst in the spot on her drivers side. As vehicle 2 pulled in it made contact with vehicle 1 and operator 1 sounded her horn and tried to notify the other operator. Operator of vehicle 1 stated she got into a verbal argument with the other operator and then vehicle 2 backed out of the spot and parked on the opposite side of the parking lot. Operator of vehicle 1 walked over and again told the other operator she hit her but again a verbal argument started and operator of vehicle 2 walked away in to the market. I was able to find the operator of vehicle 2 inside of the store and she immediately denied any wrong doing and became argumentative when I tried to speak to her. Vehicle 2 had minor damage to its front passenger side consistent with scrapes on the drivers side of vehicle 1. Operator of vehicle 2 stated her damage was old damage and that it was not scrapes on vehicle 1 but dirt and walked over and tried to rub it off with her hand. I had to advise the operator of vehicle 2 several times to return to her vehicle. I advised both operators that I would document the incident with a report and sent them on their way with no further incident. Neither vehicle required a tow and neither operator reported any injuries.

Operator of vehicle one , Allan Wint stated on 01/27/2019 at 09:40 hours he was driving his employer's pick up truck south bound on Lexington St near number 227. Lexington St is a public way in the city of Newton. Wint is a driver for Auto Zone auto parts located at 140 Lexington St , Waltham, MA ( 781 894-0656 ) . Wint was driving a 2018 Chevrolet Colorado pick up tuck ( MA reg 8CA823 ) that Auto Zone leases from Lease Plan USA Ltd. Wint stated that he was headed to Webster St in Newton to pick up a delivery. Wint stated that as he approached the area of 277 Lexington St he believed that the traffic light that is at the marked crosswalk was green for south and north bound traffic. Wint stated that he did not see any pedestrians in the crosswalk, until he heard the sound of an object impacting into the front driver's side of his vehicle. Wint stated that he stopped his vehicle approximately ten yards past the crosswalk and got out. Wint stated it was then he realized that his vehicle had stuck a female and a dog. Wint stated that he was not operating any electronic devices at the time of the crash, and I observed none in his vehicle. Wint stated he was not injured in crash. Wint's vehicle had damage to the driver's side mirror and lower molding of the driver's side front door. Wint's vehicle did not require a tow. Witness Bessy Shanks stated that she was driving in her vehicle behind Wint's vehicle south bound on Lexington St. Shanks stated that Wint's vehicle was driving slow and swerving approximately two blocks prior to the crash. Shanks stated that she believed that Wint may of been distracted. Shanks stated that south and north bound traffic was stopped because that cross walk light was red and that a female and a dog were crossing from the east to west in the marked crosswalk in font 227 of Lexington St. Shanks stated that the front driver's side of Wint's vehicle srtuck the female and the dog as they were approximately halfway into the crosswalk. Shanks stated that the female and dog both fell to the ground after the crash. Witness Elizabeth Keegan stated that she was driving in her vehicle south bound on Lexington St two vehicle behind Wint's vehicle. Keegan stated that she was not sure what color the traffic light was at the time of the crash, but traffic was stopped both not and south bound traffic was stopped. Keegan also stated that Wint's vehicle " didn't even slow down ". Keegan stated that Wint's vehicle struck the female pedestrian and the dog as they were in the marked crosswalk in front of 277 Lexington St. Witness Henry Taylor stated that his vehicle was the first one stopped at the red crosswalk light at 227 Lexington St in the north bound lane. Taylor stated that north and south bound traffic was stopped and that Wint's vehicle did not stop and stuck a female pedestrian and a dog in the crosswalk. Witness Jacqueline Deos stated that her vehicle was stopped behind Taylor's north bound on Lexington St. Deos stated that north and south bound traffic was stopped for the red light . Deos stated that Wint's vehicle did not stop and that it struck a female pedestrian and a dog in the marked crosswalk. The pedestrian in the crosswalk was identified as Xiaoping Sun. Sun stated that she and her dog stopped on the south bound side of the marked crosswalk across from 227 Lexington St. Sun stated that she activated the crosswalk light and waited until the light was red for north and south bound traffic, and that the walk single was activated. Sun stated that she looked to her left and saw traffic north bound traffic was stopping. Sun stated that she turned to her right and entered the marked crosswalk. Sun stated that she and her dog had crossed over the north bound side of Lexington St and she then looked to her right and saw Wint's vehicle not stopping and " going very fast ". Sun stated that the front driver's side mirror of Wint's vehicle stuck her in the face . Sun stated that as she fell to the ground she was struck on the left leg by Wint's driver's side. Sun stated that she did not lose consciousness after she had fallen to the ground. Sun was transported via EMS to The Brigham and Women's Hospital for treatment for head and neck injuries and abrasions to her legs. Sun's dog died as a result of being struck by Wint's vehicle. Photo's of the crash site and of Wint's vehicle were taken and the disc was downloaded by the NPD's IT Bureau. Before leaving the crash site, I tested the crosswalk lights and they were in working order. Based upon my observations of the crash site, Wint's vehicle, and the statements made to me, I concluded that Wint was at fault for this crash. I issued Wint MA Citation T 2013958 and cited him for a violation of MGL 89/11 , failure to yield to a pedestrian in a crosswalk.

MV #1 was traveling EASTBOUND on Webster St (a public way as it is maintained by the City of Newton) when it collided with a curb then a stone wall (private property belonging to 239 Webster St). MV#1 operator, Jordan Feinberg was found in front of 174 Webster St sitting on the sidewalk. He had a laceration on his arm and a lump forming on his forehead. Feinberg said that he drank half a bottle of wine tonight and hit a curb down the road (Webster St). Feinberg was sweating profusely, unsteady on his feet and had a very strong odor of an alcoholic beverage on his person. He said he was the only person in the vehicle. Officers went to go observe the vehicle and it had major front end damage and had collided with a stone wall belonging to 239 Webster St. When asked if he hit a stone wall, Feinberg said "no, I just hit a curb". He had no memory of hitting a stone wall. Resident of 239 Webster St was also the reporting party, James Pelis, said that he heard tires screeching and then a crash. He came outside and saw a MV up on the sidewalk and a male individual approximately 250lbs, heavyset, grey sweatpants and a dark shirt walking Eastbound on Webster St away from the crashed MV (this description fit Feinberg). Pelis assumed it was the driver but never saw him get out of the car. Feinberg was transported to NWH by medics. The vehicle (belonging to Feinberg's grandfather, Frederick Shapiro) was towed by Tody's. A towed motor vehicle formed was filled out. Pictures of the scene, damage to the stone wall and the MV condition were taken and the SD card was placed into IT's box. A criminal application was issued and given in hand to Feinberg at NWH by Ofc. Stake. The criminal application included 90/24J - OUI Liquor, 89/4a - Marked Lanes Violation, and 90/24E - Negligent Operation of a Motor Vehicle.

Owner of MV1 stated this morning at approximately 0830 hrs she observed damage on the rear end of her vehicle. It should be noted the owner of MV1 was not home the last couple days and observed the damage today. Owner stated she spoke to building management and was able to retrieve a video of MV2 colliding with her vehicle, which I was able to view. Owner stated MV2 which has an assigned parking spot #91 next to hers ( #89), and is seen on video on 07/25/2020 at 0942 hrs back up, attempt to turn and collide with the rear end of her vehicle. MV2 is then observed returning back to #91 at 0946 hrs. The operator and owner of MV2 is then observed exiting the vehicle and walking away. I was able to speak to owner and operator of MV2 who stated she took her vehicle for a spin around the parking lot that morning because she wanted to see if she could hear a noise coming from the vehicle that she heard the day before. Owner and Operator of MV2 stated she was not aware that she struck MV1. No injuries. MV1 sustained minor damage to the rear end. MV2 sustained minor damage to the right front end. No vehicles towed.

On 07/28/2020 at 1221hrs I responded to the area of 23 Oak St for a motor vehicle crash involving a pedestrian. The involved pedestrian had left the area prior to my arrival. The vehicle that was involved in crash was also not on scene. The crash occurred on 07/28/2020 at approximately 0900hrs in the driveway of 27 Oak St. Prior to the crash the involved vehicle described as a 1991 Chrysler TC Convertible color Red bearing MA Antique Reg D2935 ( Active) was located in the driveway of 23 Oak St. The vehicle was facing toward the rear of the driveway. The vehicle then rolled in reverse easterly across the common driveway and struck a pedestrian who was located in the driveway of 27 Oak St. After contact with the pedestrian the vehicle came to final rest in the driveway of 27 Oak St. As a result of the impact the pedestrian sustained a minor injury to her left knee. There were no occupants in the involved vehicle at the time of the crash. It is unknown at this time if the vehicle sustained any damage. The pedestrian left the area prior to this crash being reported to NPD. The vehicle was removed from the area prior to the crash being reported to NPD. I then spoke to the owner of the involved vehicle Marc Dumoulin who was on scene and was the reporting person. Dumoulin stated that the crash occurred today 07/28/2020 at approximately 0900hrs. Dumoulin states that the involved vehicle described as a 1991 Chrysler Convertible was having issues due to a gas tank problem and he believes it wasn't going to pass inspection. Dumoulin stated that he took his vehicle for a final test ride around the block to decide if he wanted to keep it or not. Dumoulin states that after operating the vehicle he decided to salvage the vehicle with CJ Auto in Watertown, MA. Dumoulin states that before going to CJ Auto he decided to stop back at his home first. Dumoulin states he then parked the vehicle at this driveway and got out to go in the house. Dumoulin states that he believes the vehicle was running at this time. Dumoulin states that as he was walking toward his house he observed his vehicle rolling in reverse towards his neighbor's driveway. Dumoulin states that the vehicle then rolled into his neighbor's driveway near his neighbors fence. Dumoulin states that the pedestrian was outside near the fence when his vehicle rolled into his neighbor's driveway.Dumoulin states that he did not initially report the crash because he did not believe that his vehicle had come in contact with the pedestrian. Dumoulin states that he then removed the vehicle from his neighbor's driveway and brought it to CJ Auto for salvage. Dumoulin states that when he returned to his residence he became aware that the pedestrian may have been struck by his vehicle and had a minor injury. Dumoulin states that upon hearing this he called NPD. At this time I was met by Nina Levin who is the homeowner of 27 Oak St. Levin stated that she did not observe the crash but a contractor working on her house did. Levin stated that the pedestrian involved was named Lucero Flores-Roman and that Flores-Roman was outside attending to some potted plants when she was hit by the involved vehicle. Levin stated that Flores-Roman received an injury to her left knee. Levin stated that she took photo's after the crash of the involved vehicle and of Flores-Roman's knee. Levin provided me with these photo's which I then submitted to the IT Bureau. Levin provided me with Flores-Romans contact phone number. Dumoulin also provided his insurance information to Levin so that she could relay the information to Flores-Roman. I then spoke with a witness Gleb Andreev who was a carpenter working at 27 Oak St. Andreev states that he was inside of 27 Oak St working on a first floor window facing the driveway. Andreev state he observed the involved vehicle roll into the driveway and come to rest near the pedestrian. Gleb stated he then went out side to assist the pedestrian. Gleb stated he believed the vehicle hit the pedestrian when it rolled into the driveway. At 0230 hrs I was able to speak with the involved pedestrian Lucero Flores-Roman. Flores-Roman states that around 0900hrs today she was in the driveway of 27 Oak St attending to some plants near a wooden fence. Flores-Roman stated that her back was turned towards the area of 25 Oak St, she then on a hunch turned around and observed the involved vehicle in reverse moving towards her. Flores-Roman stated that she turned her body quickly to avoid the vehicle but the vehicle's rear end passengers side made contact with her left knee. Flores-Roman stated that she sustained a minor injury to her left knee which included some bruising and swelling. Flores-Roman states that after crash she did not think she needed an ambulance but wanted to go home to rest her knee. Flores-Roman was then provided with Dumoulin's insurance information. No other injuries reported on scene. Photo's were taken on scene and were submitted to the IT Bureau. On 07/29/2020 I responded to CJ Auto's Salvage Yard located at 43 White Pond Rd Stow, MA which was the present location of the involved vehicle. Upon arrival I observed the involved vehicle which did not appear to have exterior damage. Photo's were taken of the vehicle and were submitted to the IT Bureau. An RMV check of vehicle revealed that a title was issued to Dumoulin on 03/25/2020. The vehicle has not been inspected by a MA inspection station. The vehicle title has been signed over by Dumoulin to CJ Auto for destruction. At this time it is unknown if the crash was caused by mechanical failure with the vehicle or human error.

While working N494 I was dispatched to a possible hit and run involving a parked motor vehicle in front of 16 Westchester Road. Upon arrival I spoke with the reporting party who stated that her neighbor saw MA REG 367NC9 back into her sons vehicle then park a few houses up on the same street. I was able to make contact with the registered owner who made contact with his daughter who was driving the vehicle. Operator of Motor Vehicle #1 stated that she was attempting to turn around pulled into the driveway and when she was backing up slowly struck Motor Vehicle #2 which was park. Operator of motor vehicle one stated that parked her car and walked over to motor vehicle #2 and did not notice any damage which is why she left not notifying anyone. I spoke to the registered owners of both vehicles and they decided to personally handle the matter. I helped both parties exchange information.

On 09/21/20 at 14:46 hours, I (Traffic Unit # 523) responded to the parking lot of 88 Chestnut Street for a motor vehicle accident involving a pedestrian. The weather at the time was 60 degrees ( localconditions.com) and the sky was clear and sunny. The road surface was dry. The parking lot is a thru way that the public use to enter/exit the lot or park there. On arrival at the same time with the Fire and Medics, I observed a female laying in the middle of the lot facing down on her back. A small red and gray CVS like type shopping carriage was knocked over and resting on it's side in front of the victim's head. The carriage was moved to the side in order to tend to the victim I also observed a black Honda Civic bearing MA reg.# 3741TW that was involved in the accident. The Honda was out of the parking spot resting approximately two feet away from the pedestrian's legs. I then spoke to the pedestrian identified as Darla Gipson in the rear of the ambulance. She sustained an abrasion to her front left shoulder and arm area which I observed. She also complained of left knee and elbow pain. She stated that she walking through the middle of the parking lot pushing the carriage by the door entrance in a westerly direction towards Chestnut Street. She states that she never saw a vehicle backing up and then she was suddenly struck and believes she was pushed underneath the vehicle. She was then transported to Newton Wellesley Hospital by Cataldo Ambulance. I then spoke to the operator of the Honda Civic identified as Linda Burke (MA S70320414). Ms. Burke was parked in the first spot to the right of the walkway that leads to the entrance/exit doorway. I asked her what occurred and she stated that she was backing her vehicle up to exit the parking lot when she looked behind her not seeing anything in sight. She then started backing up and struck the pedestrian without realizing it. She says the driver of a green cab and his female customer who were no longer on scene were yelling at her to stop her vehicle because the pedestrian was underneath it. She then stopped her vehicle. I observed minor scrapes to the entire rear bumper of the vehicle so it is unknown where it actually struck the pedestrian/carriage. I observed no obvious signs of damage to the carriage. It should be noted that Ms. Burke's Honda does not have a back up camera or back up sensors. There are no surveillance cameras to the apartment complex and no witnesses came forward for the accident. As a result of the investigation, Ms. Burke will be issued Uniform Citation # T1271069 for Newton City Ordinance 19-75 Failure To Use Care in Backing. Digital photos were taken by me of the scene.

On Wednesday, October 7th 2020 while in N526 I responded to 791 Walnut Street the Newton Cemetery Corp for a motor vehicle accident involving a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. The accident occurred inside the cemetery on the public way by the entrance. Upon arrival I spoke with the pedestrian identified as Tova Goldring. Tova stated she was walking in the roadway when she was struck by MV1. Tova stated MV1 was turning left onto the road that leads to the exit and when she realized the operator was not stopping she put out both her hands and made contact with the hood of MV1. Tova stated the impact caused her to fall to the ground and roll into a grassy area next to the road. Tova had minor scrapes from the accident and signed a patient refusal. The operator of MV1 identified as Antonio Domenico Ricci was on scene and his primary language was Italian. I was able to contact his daughter, Daniela Boccia who arrived on scene and translated Antonio's statements. Antonio stated he was turning left onto the main road that leads to the exit of the cemetery and had his left signal activated. Antonio states the sun was glaring into his eyes and made it hard for him to see. Antonio stated he took the left turn and never saw the pedestrian. Antonio states he heard a thud on his hood and stopped the vehicle and that is when he realized he had struck a pedestrian. Antonio was not injured and there was no damage to his vehicle. Pictures of the scene and Tova's injuries were taken by Officer Wilson and submitted to the IT Bureau. Daniela took possession of MV1 and drove Antonio to his residence.

On 10/22/20, while assigned to N499, I, Officer Conary, responded to 80 Redwood Road, for a report of a single vehicle accident into a tree. Upon arrival, I met with the Operator of MV's husband who was standing by at the car. Operator of MV1 walked home to contact her insurance company but she was on her way back to the accident. Operator of MV1 explained to me that she was traveling Northbound on Redwood Road to her house when she was adjusting the air conditioner. She stated she looked down twice to adjust it and then she hit the tree. Operator of MV1 declined medical attention. MV1 had heavy front end damage and was towed by Tody's. There was minor damage to the City of Newton tree. Pictures were taken and submitted to IT accordingly. City of Newton DPW was notified of the debris in the road and sidewalk.

Single MVA. MV lost control on the roundabout on RT 16 Washington St heading EASTBOUND, hit an island and was laid to rest on the sidewalk of the MassPike bridge on Rt-16. Heavy front end damage, airbag deployment, undercarriage and front wheels damaged as well. A witness notified Lt. DAngelo in front of NPD HQ that there was a crash and two occupants got out, a man and a woman. Lt. DAngelo was able to locate the male walking on Washington St who was involved in the crash, later identified as Jonathan Kardas. He stated that he was the passenger and his friend who was driving the vehicle but ran away. Multiple units looked for the female individual who was Sonia Walker, the owner of the MV. Kardas signed a refusal with the medics. Tody's towed the vehicle. Sonia's Iphone was secured into property locker #2 in support services for safekeeping. Dispatch made multiple attempts to contact Walker but had negative results.

On 12/3/2020 at 1400hrs I investigated a past motor vehicle crash involving a pedestrian. The crash occurred on 12/03/2020 at approximately 1240hrs. The crash was located in the area of 483 Centre St. Centre St is a public way in the city of Newton. Vehicle 1 was traveling southbound on Centre St near the area of 483 Centre St. Pedestrian 1 was located on the southbound side of Centre St near 483 Centre St. Pedestrian entered the southbound lane of Centre St and was struck by Vehicle 1. As a result of the impact Pedestrian 1 sustained a minor injury to his left leg. Vehicle 1 had no known damage. No parties were transported. No tow was required. It should be noted that the crash was reported by the Operator of Vehicle 1 Daniel Zandman at approximately 1400hrs. Zandman states that he was operating his Vehicle southbound on Centre St just after the intersection at Church St. Zandman states that Pedestrian 1 unexpectedly ran in front of his vehicle. Zandman stated that he did not have enough reaction time to come to a complete stop and his front bumper made contact with Pedestrian 1. Zandman stated that after impact he immediately got out of his vehicle and checked on Pedestrian 1. Zandman described pedestrian on as a 10 year old white male identified as Dexter Jagoditish. Zandman stated that while on scene Dexter called his mother Jess Jagoditish who immediately responded to the scene. Zandman stated that he exchanged information with Jess Jagoditish who then took Dexter home. Zandman states that a witness Mia Martin was also on scene. At approximately 1425hrs I called and spoke to Dexter and his mother Jess Jagoditish. Dexter states that he was walking on the sidewalk near 483 Centre St. Dexter states that he wanted to cross Centre St and thought there were no cars coming. Dexter stated that he entered the southbound lane of Centre St and a car came out of no where and hit him. I then spoke to Jess Jagoditish who is Dexter's mother. Jess stated that she received a call from Dexter around 1240hrs stating he was just hit by a car. Jess stated that when she arrived on scene Dexter was with Zandman and a witness Mia Martin. Jess stated that both Zandman and Martin explained them what had happened. Jess stated that Dexter appeared to be ok so she decided to take him home. Jess states that at this time Dexter has a bruise on his left calf but otherwise appears to be ok. At 1551hrs I spoke with the witness Mia Martin. Martin states that just prior to the crash she was walking her dog behind Dexter. Martin states that "out of nowhere" Dexter sprinted into the southbound lane of Centre St. Mia states that she then observed Vehicle 1 attempt to slow down and swerve to the lest to avoid Dexter. Mia states that the passenger side front bumper then made contact with Dexter. Mia states at impact Vehicle 1 was traveling at a very slow speed. Mia states that Dexter was then knocked on to the ground but then immediately jumped up and started to run away. Mia states that both she and Zandman were able to get Dexter to comeback so that they could check to see if he was ok. Mia states that Dexter was then able to contact his mom who responded on scene. Due to this crash being reported after the fact, Fire, EMS or Police where not initially notified and did not respond to the scene. Pictures were also not taken of the crash scene. At this time this report is for documentation purposes. After speaking with all the parties involved it appears that the crash was caused by Pedestrian 1 entering the roadway in a manner which gave Vehicle 1 little or no time to react.

At approximately 1655HRs on Monday, January 25, 2021, I was dispatched to the 15 Comm Ave. for a report of an MVA, in which the offending vehicle left the scene. Upon arrival, I spoke with the owner of MV1 (NH Pass: 2873301), who stated that his coworker ( Witness 1) observed MV2 hit his vehicle and leave. To this end, he showed me the resulting damage to vehicle's rear bumper, as well as a photograph Witness 1 took of MV2. Boston Police located MV2 at it's listed garaged address. Upon speaking with the owner/operator of MV2 (MA Pass: 17MA21), he stated that he was in the area, but denied hitting MV1. I informed him of the witness statements and asked him about the damage to his vehicle, but he again stated that he did not believe he was involved, yet could not account for the damage. Upon speaking with Witness 1, he stated he was walking Westbound on Comm Ave. at approximately 1650HRs, when he observed M2 attempt to exit a parking spot in front of 15 Comm Ave. (MV2 was parked behind MV1). MV2 then scrapped MV1 while attempting to do so, causing MV1 to rock forward. Witness 1 then took a picture of MV2's rear license plate. I observed scratches to the rear driver's side bumper area of MV1 and scratches to the front passenger's side bumper area of MV2, along with a slight separation of the bumper cover of MV2 and it's mounting points on the same side. Both vehicle's damage was consistent with Witness 1's account of a vehicle scraping a parked vehicle, as it attempted to exit a parking spot from the rear. It should be noted that the damage to both vehicles appeared minimal and it is plausible that the operator of MV2 may not have known about the collision. As a result, the operator of MV2 will not be charged with a hit and run accident at this time. I took pictures of both vehicle's damage and they, along with Witness 1's picture will be submitted to IT to be attached to this report.

The owner of MV#1 stated she had parked her vehicle in front of 50 Union St on Saturday 03/13 and discovered a hand written note on her windshield. The note was from a party who witnessed MV#2 struck MV#1. MV#1 sustained minor damages to its driver's side door (Lower panel). I spoke to the witness who stated on Saturday 03/13 at 14:30 he was sitting outside at 45 Union St (The Bar-A-Mor Restaurant) and observed MV#2 attempting to parallel park behind MV#1. The witness stated he heard a loud bang and observed MV#1 shook. The witness then observed 2 Asian parties (1 male party and 1 female party) exit MV#2 while carrying carpet/ rugs walking northbound on Langley Rd. The witness stated he was unsure if the male or the female party was operating MV#2. The witness described MV#2 as a black colored "BMW X5" SUV bearing New York registration GFS4413. The witness stated he was unsure what part of MV#2 made contact with MV#1 but believes it to be either its front passenger side tire area or rear passenger side tire area. I spoke to the operator of MV#2 and she stated she is the owner of a BMW X5 SUV bearing New York registration GFS4413 and was in Newton Massachusetts on Saturday 03/13 delivering some carpet/ rugs to a nearby business. The operator of MV#2 stated she was unaware she had made contact with any vehicles. The operator of MV#2 was forthcoming with her information and provided her driver's license number along with her insurance provider. Based on the statements made to me, I will not pursue this accident further. The operator of MV#2 was provided with this accident report number and advised to contact her insurance provider.

On 03/30/21 at 09:58 hours, Ofc. Marini (N537) and I (Ofc. Wilson (N523) responded to the parking lot of Tedeschi's located at 612 Washington Street for a two car motor vehicle accident involving a city motor vehicle which belonged to the Newton D.P.W.. At the time of the accident, weather conditions were clear and sunny with dry road conditions. The operator of M/V # 1, Danielle Finklea MA D/L S09835151, states she was eastbound on Washington Street in her 18' Mitsubishi Outlander (MA reg. # 9TM745) and turned right in to the parking lot of Tedeschi's. While pulling in, Finklea misjudged the distance that M/V # 2 was parked in a spot to the left of her. She then struck the right side of M/V # 2's rear bumper which sustained scrape marks to it. The Outlander sustained scrapes to the driver's side front quarter panel area and a broken headlight lens cover to that same side. The other vehicle (M/V # 2) was a 2014 Ford F350 pickup bearing MA Official Reg.# M87258 which is owned by the City of Newton Department of Public Works. At the time, the F350 was parked in a parking spot in front of the store unoccupied. The operator, Robert Mazzarella, stated he saw the F350 shake as it had been struck so he walked over to the other operator involved. No injuries reported. Ofc. Marini took digital photos of the scene and submitted them to the I.T. Bureau.

Operator #1 states that she was traveling northbound by 226 Lexington St when she observed vehicle #2, a white Ford Explorer, approaching at a high rate of speed from Lexington St southbound. Vehicle #2 then crossed the double yellow line into the northbound lane, subsequently sideswiping vehicle #1. Operator #1 states that two parties exited the Explorer and spoke briefly to her. Once she requested to exchange information, the two refused and drove away on Lexington St southbound. Operator #1 described the parties as a hispanic male and hispanic female, around 50 years old. The male party was balding/or bald, and the female was wearing a silver turtleneck. Operator #1 noted that it was the male who was operating the vehicle both during the collision and as the vehicle left the scene. While medics were documenting a patient refusal for operator #1, Ofc Ciccone was en route to NPD headquarters to report for duty in his p/v and heard the broadcast for the suspect vehicle. Ofc Ciccone observed a white Explorer with a flat tire traveling an estimated 50+ mph past him on South Ave by Park Rd in Weston heading towards Natick. This location is approximately 2.2 miles from the crash site and is accessible from 226 Lexington St by heading southbound, taking a right, and continuing straight on Rt 30. Weston Pd was immediately notified and the vehicle was located in a driveway at 7 High Meadow Rd, a residence directly on the corner of Rt 30. Sgt Rooney and Ofc Ciccone (who was able to take a spare cruiser and call into service as this incident unfolded) responded and met Weston PD. While on scene, it was explained to all officers by the resident of 7 High Meadow Rd, Helder Quesada, that he was walking his dog and observed the white Explorer broken down in an unsafe spot. Quesada offered up his driveway to park the vehicle as the male and female occupants waited for assistance. Quesada allegedly does not know Lopez and Albizureis, and all involved persons were outside in the driveway when police located the vehicle. The description of the male asociated with the Explorer matched that which operator #1 gave to me- a hispanic male balding in his 50's. He was in the presence of a hispanic female who was wearing a silver colored turtleck. The male was identified as Armando Lopez-Rubi DOB 1/1/73 and the female Ana Maria Albizureis DOB 11/27/73. The Explorer was MA reg 6GZ591, registered to a different party then all on scene. It had obvious damage to the left side doors consistent with an opposite direction sideswipe. With the translation assistance of Helder Quesada, Lopez-Rubi stated to Sgt Rooney and Officer Ciccone that he was in fact driving somewhere near Starbucks "on the corner" when he got into an accident (there is a large Starbucks located on the corner of Commonwealth Ave and Lexington St in Newton). Lopez originally said that the other involved vehicle left the scene, however it was pointed out by Sgt Rooney that the operator was currently stopped in Newton talking to me and was cooperative. Upon routine query it was discovered that Lopez-Rubi does not have a license to operate a motor vehicle. Pursuant to the investigation, Officer Ciccone issued in hand to Lopez MA citation R8006998 for the following: 89/4a- Marked Lanes Violation 90/24- Leaving Scene of Property Damage 90/10- Unlicensed Operation of a Motor Vehicle. Vehicle #2 was left in the driveway of 7 High Meadow Rd with Helder Quesada's permission. Quesada also volunteered to drive home both Lopez and Albizureis to Framingham, as he was feeling charitable. Vehicle #1 was towed by Tody's due to the amount of damage and airbag deployment. A m/v inventory form was filled out.

On Thursday May 13th, 2021 at approximately 1353 hours myself and several units responded 55 Chapel Street for a motor vehicle that struck a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. Chapel Street is a public way owned and maintained by the City of Newton. Upon arrival Fire and medical personnel were attempting to remove the pedestrian from underneath the vehicle as her right leg was stuck in the undercarriage. Fire and medics were able to successfully free the pedestrian from the vehicle and render medical attention. The pedestrian was identified as Beth Herr ( S58524067) and it was determined that she was also the owner of the vehicle involved. I spoke to witness Joe Shields who stated he was outside his house located at 56 Chapel Street when his attention was brought to a blue SUV that was unoccupied moving backwards down Chapel Street. Joe stated he first witnessed the operator of the motor vehicle pull up to the UPS drop box located on the sidewalk across the street from his house. He stated he saw the female operator step out of the vehicle and drop off a package. Next he saw the vehicle start to roll backwards and the female operator opened the drivers side door and attempted to pull the emergency break. He stated the vehicle overtook her and she fell to the ground with her foot stuck under the car. Joe states he heard her yell and saw her get dragged by the vehicle to the opposite side of the street where the vehicle was stopped by the curb. Joe stated he tried to assist the woman but she was yelling in pain and he did not want to cause more injury so he waited for help to arrive. Witness Robyn Preston stated she was walking on Chapel Street when she saw the blue SUV rolling backwards with no one inside the vehicle. She states she then saw a woman try to stop the vehicle but she got dragged underneath the vehicle to its final resting place. Robyn stated she stayed on scene until help arrived. I was able to speak with Beth inside the ambulance prior to being transported to the hospital. Beth was conscious and alert but in a considerable amount of pain and discomfort. I asked her what happened to which she stated she drove her blue Toyota Rav4 MA REG: 1FHM53 to the UPS drop box located in the area of 55 Chapel Street. She stated she thought she put the vehicle in park but must not have as it started to roll backwards when she got out of the car. She states she tried to stop the car by opening the drivers side door and attempting to pull the emergency break. The car over took her and she was dragged to the ground with her right foot stuck underneath the vehicle. She was dragged across the street where the vehicle was finally stopped by a curb. Beth sustained minor scrapes and road rash on various parts of her body and her right leg was injured. Fallon medics transported her to the Brigham & Womens Hospital to be treated. Her husband was contacted using Beth's cell phone and notified of the accident. Captain Boudreau and I were able to drive her vehicle to her residence at 50 Broadway Street, Newton. Her vehicle was left parked outside her house and her car keys were left with her husband who was inside the residence. The vehicle did not sustain any damage. Pictures of the scene were taken and submitted to the IT Bureau. I will be following up with Beth at the hospital to check on her status. On Friday May 14th at 0900 hours I spoke with Beth on the phone. She stated she is home from the hospital and doing much better. She states she has right leg pain and road rash from the accident.

On Monday, May 17, 2021 I responded to the parking lot entrance of the Avalon at Chestnut Hill (160 Boylston Street, Newton) for a report of a motor vehicle/pedestrian crash. The weather at the time of the crash was clear and sunny. The road surface was dry. Boylston Street is a public way in the City of Newton. I spoke with the operator of MV1, Mr. Joshua Champion (SA1310472). Mr. Champion stated he was operating his 2013 Lincoln MKS (MA: VT41980) while attempting to exit the Avalon parking lot at the Boylston Street (E) off ramp. Mr. Champion stated he was monitoring the traffic to merge and as he started to make his right turn his vehicle crashed into the left side of a pedestrian in the roadway. Mr. Champion stated he did not see the pedestrian as he was making his turn. I observed no damage to the area where the pedestrian made contact with the car. Photos were taken and submitted to the IT Bureau. The injured pedestrian, Mrs. Joan Contompasis, stated she was walking on Boylston Street (W) with her husband. Mrs. Contompasis stated she entered the roadway from off the sidewalk to cross the Avalon parking lot exit and continue on Boylston Street. Mrs. Contompasis stated she believed MV1 had stopped to allow her and her husband to cross. Mrs. Contompasis stated the vehicle then accelerated and it's front passenger side bumper area crashed into her left side. Mrs. Contompasis fell to the roadway. Mrs. Contompasis was transported to Newton Wellesley Hospital for injuries to her right foot and knee. I spoke with a witness to the crash, Mr. Michl Contompasis. Mr.Contompasis stated he was walking with his wife Joan on Boylston Street (W). Mr. Contompasis stated he was walking just ahead of his wife as they crossed the Avalon at Chestnut Hill exit at Rt. 9. Mr. Contompasis stated he observed a black sedan to his left as he crossed. Mr. Contompasis stated he believed the operator was not paying attention as he exited the parking lot and the vehicle accelerated into his wife's left side.

MV1 was traveling Southbound while MV2 traveled Northbound on Lowell Ave. Around the intersection of Foster St at Lowell Ave, MV2 hit MV1's driver's side mirror with their driver's side mirror. MV1 side mirror was smashed and glass was inside the vehicle and the operator came in contact with some of the glass due to the driver's side window being open during the time of the collision. MV1 operator, John Mole, spoke with me at his residence after the accident because the operator of MV2 drove away from the scene without giving their information. MV1 operator had some dried blood on his left hand and left leg from the glass of the side mirror but refused medical treatment. He stated that after the crash, both MVs pulled to the side and operator of MV2 came out and said he didnt have his license and said he was in a hurry for an appointment and drove off. MV1 operator was able to read and write down MV2 license plate before it left the scene. MV1 operator describes MV2 operator as a tall 6'2" African male with short hair. MV2 was a company van belonging to MAB Community Services Inc. Traffic Bureau update (Officer Gaudet): I contacted MAB Community Services, Inc. to identify the operator of MV2. Heather Ripley, Director of Residential Services at MAB Community Services stated the operator of MV2 is Mr. Moses Mubiru. Ms. Ripley stated Mr. Mubiru provided them with a picture of the other vehicle involved in the crash and stated the other operator would not exchange information with him. On Thursday, May 27, 2021 I spoke with Mr. John Mole. Mr. Mole stated he was traveling South on Lowell Avenue towards Walker Street. Mr. Mole stated MV2 was traveling North on Lowell Avenue towards Washington Street. Mr. Mole stated as the vehicles passed each other their driver door mirrors collided. Mr. Mole stated both parties pulled over to assess the damage. Mr. Mole stated the operator of MV2 stated he did not have his license with him and it was a company vehicle so he did not have the registration. Mr. Mole stated the operator of MV2 stated, "I have to go I have an appointment, I have to pick someone up." Mr. Mole stated the operator of MV2 took a picture of his license plate and left the area without attempting to exchange information. I then contacted Mr. Moses Mubiru (S83293592). Mr. Mubiru acknowledged he was the operator of MV2 at the time of the crash. Mr. Mubiru stated he was operating MV2 South on Lowell Avenue towards Washington Street when his driver door mirror collided with the driver door mirror of MV1. Mr. Mubiru stated he did not exchange information with the operator of MV1 because that operator was unwilling to exchange information. Mr. Mubiru stated he took a picture of MV1's license plate and reported it to his employer. Mr. Mubiru's information was added to this report to reflect him as the operator of MV2.

On Monday, June 14, 2021 while assigned to Traffic unit N525, I responded to the intersection of Park Street and Alba Circle for a report of a motor vehicle crash involving a pedestrian. The weather at the time of the crash was overcast with light rain. The road surface at the location of the crash was wet. Park Street and Alba Circle are public ways maintained by the City of Newton. Park Street supports two way travel (N, S) with no marked divider to indicate travel lanes. I spoke with the operator of the 2013 Nissan Juke (MA: 137LX7) involved in the crash, Ms. Cynthia Cowan (S59504510). Ms. Cowan stated she was traveling on Park Street (N) towards Alba Circle. Ms. Cowan stated ahead of her there was a large landscaping truck (MA CON: X1515, 2001 Ford DRWSUP) parked on the right side of the roadway and blocking her lane of travel. Ms. Cowan stated she looked ahead of the landscaping truck and did not see any traffic coming towards her in the opposite lane. Ms. Cowan stated she entered the opposite lane of travel (S) to pass the parked landscaping truck and continue down Park Street. Ms. Cowan stated as she was traveling past the front of the landscaping truck, she crashed into a standing mower that had entered the roadway. Ms. Cowan stated the mower entered the roadway from in front of the parked landscaping truck. Ms. Cowan stated because of her obstructed view from the landscaping truck, she did not see the mower until it was too late. Ms. Cowan stated she stopped her vehicle and observed the operator of the mower on the sidewalk in front of 248 Park Street. I observed damage to the front passenger fender/bumper/wheel area with airbag deployment from the driver seat. I also observed yellow paint transfer on the front bumper area consistant with the color of the mower involved. MV1 was removed from the roadway by Tody's Towing. Ms. Cowan signed a patient refusal with Newton Medics and walked home from the crash site. The other party involved in the crash is a landscaper for 4 Seasons Services. I observed a yellow Wright Standing Mower 52" resting on the side of the roadway in the Northbound lane. The mower has significant damage to its front portion. 4 Seasons Services removed the mower from the roadway. A worker on scene estimated the value of the mower to be $11,000.00. Photos were taken of the mower, MV1, and the roadway and submited to the IT Bureau. The injured party, Mr. Louis Padilla-Gonzalez (S32927586), was transported to Newton Wellesley Hospital for a left shoulder injury and an injury to the left side of his face. I spoke with Mr. Padilla-Gonzalez in the emergency room with the assitance of a language translation service set up by the nursing staff there. Mr. Padilla-Gonzalez stated he was operating the standing mower and left the sidewalk into the road. Mr. Padilla-Gonzalez stated he drove by the front of the parked landscaping truck and when he got into the street he saw a car coming towards him fast. Mr. Padilla-Gonzalez stated he tried to reverse the mower and get out of the way, but he could not because of the wet roadway. Mr. Padilla-Gonzalez stated he was hit by MV1 and thrown from the mower to the street. After speaking with Ms. Cowan and Mr. Padilla-Gonzalez, it appears both parties had their view of the roadway obstructed due to where the 4 Seasons Services landscaping truck (MA CON: X1515) was parked. I did not observe any posted parking restrictions where the vehicle was parked, but due to the size of the vehicle it did take up the entire Northbound travel lane in front of 248 Park Street. The location of the parked landscaping truck and the wet road surface contributed to the crash. There were no witnesses to the crash.

On Monday, June 28, 2021, while assigned to Traffic unit N525, I was entering my marked cruiser to leave the parking lot at the rear of 1321 Washington Street, Newton. The rear parking lot of the police station features a public right of access, but is restricted for authorized vehicle. There are signs posted at both entrances to the parking lot stating, "RESTRICTED AREA Newton Police Department Authorized Vehicles Only Beyond This Point. As I entered my cruiser, I observed a red 2020 Jeep Wrangler (CT: AX89516) parked directly across from my cruiser facing Eastbound. This vehicle was not parked in a marked parking spot and was the last vehicle in the parking lot from the Cherry Street entrance on the North side of the lot. I observed that the front bumper of this vehicle made contact with the rear bumper area of a parked 2018 Tesla Model 3 (EV3617 ). I exited my vehicle and walked over to the vehicles to investigate if a crash had occurred. I observed a black male party sitting in the front passenger seat of the Jeep. The driver seat was vacant. The Jeep had its driver door and passenger door windows rolled down. The vehicle was not running and there were no keys in the ignition. As I approached the passenger side door, the male passenger appeared to be asleep and I could detect a moderate odor of alcoholic beverage emanating from the passenger seat area. I woke up the passenger and inquired where the driver of the vehicle was. The passenger identified himself by his Connecticut ID as Masengo Kabongo (#178372788). Mr. Kabongo stated his cousin Marcel was the operator of the vehicle and they had pulled into their current parking spot so Marcel could run an errand. Mr. Kabongo stated he did not know where his cousin was, and he could not locate his phone to contact his cousin. I asked Mr. Kabongo if he was aware he was parked in the Newton Police Station parking lot and that the vehicle he was sitting in crashed into an employee's vehicle parked in front of them. Mr. Kabongo stated he was not aware. Mr. Kabongo appeared to be very confused with the situation and continued to state he did not know why they were there and where his cousin was. The registered owner of the Tesla arrived and assessed the damage to his vehicle. I observed minor damage to the rear bumper area to MV2. There was also minor damage to the front bumper of MV1. The owner of MV2 was able to review footage of the crash from his vehicles exterior surveillance system. While viewing this video with him, I observed MV1 enter the Newton Police Department parking lot from Cherry Street and pull right behind MV2. MV1 appeared to be occupied by two black males at this time. One in the passenger seat and one in the driver seat. MV1's front bumper area is then observed crashing into the rear bumper area of MV2. The Newton Police Department IT Bureau was able to observe the crash from a surveillance camera affixed to the Newton Police Department maintenance garage (facing Cherry Street). The video captures MV1 entering the parking lot and crashing into the rear of MV2. A black male with light colored shorts and a dark T-shirt is observed exiting the driver side door and walking away from the area up Cherry Street (S) to Washington Street. I explained to Mr. Kabongo that the crash was captured on our surveillance system and that his cousin appeared to be the operator. Mr. Kabongo stated his cousin was currently in Newton District Court being arranged for a previous incident (Report #21027283). I looked up recent bookings for NPD and I observed a party matching the name Mr. Kabongo provided. On Friday, June 25, 2021. Mr. Marcel Mengue-Ngoumou was arrested and cited by the Newton Police Department for OUI (Subsequent Offense), Negligent Operation, Marked Lanes, Open Container, and Unlicensed Operation. I contacted the Newton Police Department Court Prosecutor, Officer Hagopian, to see of Mr. Mengue-Ngoumou was still present in Newton District Court. Officer Hagopian stated he contacted the court and Mr. Mengue-Ngoumou was waiting to be arraigned. Myself and Officer Hagopian responded to Newton District Court to try and speak with Mr. Mengue-Ngoumou. Upon entering Session 1 in NDC, the lone individual sitting in the gallery was a party matching the description of the party from the surveillance footage. I spoke with Assistant District Attorney Nathan Stetson who was present in the court room. I apprised him of the current incident that Mr. Mengue-Ngoumou was involved in and told him I was going to speak with him. Mr. Mengue-Ngoumou at this time was sitting on the second to last bench on the left side of Session 1 as you walk into the courtroom. The exit to the courtroom was behind him. I approached Mr. Mengue-Ngoumou from the front of the courtroom and explained to him that I had just spoken to his cousin in the parking lot of the Newton Police Department. I explained to him the vehicle his cousin was sitting in had crashed into a parked vehicle belonging to a Newton Police Department employee. I explained to Mr. Mengue-Ngoumou that his cousin had identified him as the operator of the vehicle. I then asked Mr. Mengue-Ngoumou who was operating the vehicle when they were traveling to court today. Mr. Mengue-Ngoumou stated he was the operator. I asked Mr. Mengue-Ngoumou if he knew he had parked in the Newton Police Department parking lot. Mr. Mengue-Ngoumou stated he did not. I asked Mr. Mengue-Ngoumou if he knew he crashed into a parked vehicle in the parking lot. Mr. Mengue-Ngoumou stated he did not. I asked Mr. Mengue-Ngoumou if he knew he was not supposed to be operating a vehicle because of his incident from the other night and his license being suspended. Mr. Mengue-Ngoumou stated he did. I apprised ADA Stetson to my conversation with Mr. Mengue-Ngoumou. Mr. Mengue-Ngoumou was issued Massachusetts Uniform Citation 054528AB in hand for Chapter 90, Section 23(K) (Operating a MV after suspension, subsequent offense), and Chapter 90, Section 24 (Leaving the Scene of Property Damage). Mr. Mengue-Ngoumou knowingly crashed into MV2, parked his vehicle, took the keys from the ignition, and left the area with out making an effort to contact the owner of MV2. The 2020 Jeep Wrangler from the crash was removed from the Newton Police Department parking lot by Tody's towing. A towed MV form was completed and submitted. Washington Street and Cherry Street are both public ways maintained by the City of Newton.

There was a medical emergency at 2101 Washington Street (Care One) so Fire, Medics, and Police were all parked by the front entrance. The OP. of MV#1 states that she was trying to park correctly in her spot. She backed out and her attention was drawn to an ambulance and didn't realize there was a second ambulance behind her. That's when she collided with MV#2. MV#2 is an unoccupied ambulance with its emergency lights on while medics were out assisting a patient. Medic J. Spratt was nearby and observed his truck shake and walked around and noticed MV#1 pulling away from the ambulance with damage to the corner of the vehicle. Pictures have been taken of both MV#1 and MV#2. Ambulance MV#2 has some old damage not related to this current accident. There doesn't appear to be any new damage to MV#2 from this current accident.

On Thursday 7/15/21 at approximately 0900hrs I was dispatched to 12 Rockland Street for the report of a motor vehicle with heavy damage resting in the middle of the street. Upon arrival I observed a brown Nissan Altima unoccupied not running in the middle of street. Heavy front end damage on the passenger side along with two flat tires on the passenger side. There was no operator on scene but the car was registered to 12 Rockland Street. It should be noted that Rockland Street is a public way located in the City of Newton. I spoke to the reporting party Barry Cocuzzo who lives at 18 Rockland Street. Mr. Cocuzzo stated he witnessed William Power operating the Nissan and turn onto Rockland Street. Mr. Cocuzzo observed Power drive into his driveway with the two flat tires and heavy front end damage. When Power exited the vehicle Cocuzzo yelled over to him but stated Power's was unable to communicate clearly to him. Cocuzzo observed Power's walk into his residence at 12 Rockland and then bereaved the vehicle slowly roll back out of the driveway. Medic 3 and Tody's responded to the scene. Due to the heavy damage and two flat tires Tody towed the vehicle. A towed motor vehicle inventory report was filled out. Medic 3 was called to the scene to evaluate the health of Power's. Officer Tocci and myself located Power's in the residence passed out on the couch. Empty alcohol cans were all around Power's. After several attempts to wake up Power's he finally came too. Power's speech was severely slurred, he could barely keep his eyes upon and he lacked the ability to stand up without assistance. The Medics transported Power's to Newton Wellesley. Power's is being cited (Ma Citation 069847AB and 06850AB) for 90/10/A Unlicensed Operation, 90/24/E Negligent Operation of MV, 90/24/C Leave Scene of Property Damage, 90/24/J OUI Liquor and 90/24/P Use MV Without Authority. 5 photos were taken of the motor vehicle. It should be noted that the original location of the crash and the object that was struck are unknown to us at this time. Dispatch was also able to confirm that Power's did not have an active license at this time. Power's mother Bernice Fougere who is the owner of the vehicle stated to me that Power's did not have permission to use her vehicle.

There I met the operator of MV1 show stated that he accidently struck a city fire hydrant with his company truck (Verizon). The hydrant itself did not appear damaged. However, it was dislodged from the side walk and no longer secured by its bolts. The Newton water dept was notified and responded.

On Monday, July 19, 2021, while assigned to Traffic unit N525, I responded to the area of Commonwealth Avenue and Lowell Avenue, Newton for a report of a MVA/Pedestrian crash. Commonwealth Avenue and Lowell Avenue are both public ways maintained by the City of Newton. The weather at the time of the crash was overcast with light rain. The road surface at the time of the crash was wet. Feeney Brothers Utilities has been performing gas repair services at this intersection for the past month and they have had a paid Newton Police Detail with them during their service. The detail presented itself as follows on my arrival. There was service being conducted in the northbound lane of Lowell Avenue in-between the Commonwealth Avenue carriage roads. A large trench was present in the northbound lane that made the roadway impassible. Traffic was permitted to travel down the southbound lane of Lowell Avenue towards Commonwealth Avenue for the detail. Access to Lowell Avenue from Commonwealth Avenue was prohibited. The northbound lane was blocked by orange construction cones and road closed signs. There was also no access to the Commonwealth Avenue carriage roads at this location. Both of these roadways were closed with orange construction cones and caution tape. There was also caution tape and orange construction cones restricting access to a walkway that traveled from Commonwealth Avenue to its carriage road just after Lowell Avenue (E). I also observed a "Sidewalk Closed" sign posted on the walkway. I spoke with Officer Gabriel of the Newton Police Department. Officer Gabriel stated he was working a paid detail for Feeney Brothers at this location. Officer Gabriel stated he had just assisted two female parties cross Commonwealth Avenue (E) at Lowell Avenue. Officer Gabriel stated a short time later, he heard a scream and observed two female parties laying on the roadway on the opposite side of Commonwealth Avenue in the carriage road. Officer Gabriel immediately contacted Newton Police Dispatch via his portable radio and requested aid for the injured parties. Prior to my arrival, Officer Gabriel stated one of the pedestrians was transported to Newton Wellesley Hospital with a head injury. I spoke with one of the pedestrians involved in the crash, Ms. April Stein. Ms. Stein stated she was walking in the carriage road of Commonwealth Avenue (E) towards Lowell Avenue with her friend Sarah Kish. Ms. Stein stated when they got to Lowell Avenue there was work being done in the roadway, and a Police Officer assisted them with crossing the area. Ms. Stein stated they then re-entered the carriage road when they crossed Lowell Avenue. Ms. Stein stated they were walking along the edge of the roadway when they were hit by a front loader. Ms. Stein stated they fell to the roadway due to the collision. Ms. Stein stated she did not hear or see the front loader backing up towards them and did not notice it because they were having an "intense conversation". Ms. Stein reported no injuries and signed a patient refusal with Fallon Medics. I asked Ms. Stein if she had transportation to her residence. Ms. Stein stated she would walk home. I spoke with the operator of the front loader involved in the crash, Mr. Mark Mannion (S91049325). Mr. Mannion is an employee of Feeney Brothers Utilities and stated he was operating a 2015 Case Loader (MA CON: R99118) at the time of the crash. Mr. Mannion stated he was backing up the loader down the Commonwealth Avenue carriage road (E) from Lowell Avenue. Mr. Mannion stated he was going to get a load of fill from a pile of dirt on the side of the road inside the marked off work area. Mr. Mannion stated as he was backing up, he was looking over his right should behind him to make sure there were no obstacles in his way. Mr. Mannion stated his emergency backing alarm was activated while he was backing up. Mr. Mannion stated he noticed something enter the roadway to his right side and immediately slammed on his brake. Mr. Mannion stated then heard a scream and saw 2 female parties on the ground. Mr. Mannion stated he exited the loader and checked on the welfare of the two women. Mr. Mannion's CDL License, Vehicle Registration, and Hoisting License are all active. I asked Mr. Mannion to put the vehicle in reverse to assess if the emergency backing alarm was functioning properly. Mr. Mannion was cooperative and while reversing the vehicle the emergency backing alarm was functioning properly in my presence. I asked Mr. Mannion if the loader had moved since it was brought to a stop due to the crash. Mr. Mannions stated it had not moved. I observed this vehicle to be within the parameters of the work zone that was taped off. The vehicle at this time was released back to Feeney Brothers. Photos were taken of the work site and loader and submitted to the IT Bureau. I responded to Newton Wellesley Hospital and spoke with Ms. Sarah Kish (S85088490) in the Emergency Room. Ms. Kish stated she was walking down the Commonwealth Avenue carriage road (E) towards Lowell Avenue with her friend April Stein. Ms. Kish stated a Police Officer assisted them with crossing Lowell Avenue via the crosswalk located on Commonwealth Avenue. Ms. Kish stated after they crossed Lowell Avenue, they walked to the carriage road. Ms. Kish stated as they were walking, she was hit from behind by a front loader in the roadway. Ms. Kish stated she did not hear or see the front loader and believes the bucket hit her in the head. Ms. Kish stated she fell to the roadway. Ms. Kish was transported to Newton Wellesley Hospital for her injuries. Ms. Kish stated at this moment, she had some lacerations to her back and head, and her left shoulder/neck/back area was sore. I asked Ms. Kish how often she walks that particular route. Ms. Kish stated 5 times a week. Ms. Kish stated there was nothing in place to restrict her access down to the carriage road after she crossed Lowell Avenue. I spoke with a witness to the crash via telephone, Ms. Megan Boitano. Ms. Boitano stated she was traveling on Lowell Avenue (N) towards Commonwealth Avenue. Ms. Boitano stated she was stopped at the light right next to the trench that was in the northbound lane of Lowell Avenue. Ms. Boitano stated she observed two women crossing Lowell Avenue at Commonwealth Avenue in front of her. Ms. Boitano stated she then observed a heavy piece of construction equipment approaching the trench to her left at a high rate of speed. Ms. Boitano stated it startled her as the front loader dumped dirt into the trench. Ms. Boitano stated she then observed the front loader back down the carriage road at a high rate of speed towards a dirt pile. Ms. Boitano stated she observed the two women that just crossed Lowell Avenue in the roadway behind the front loader. Ms. Boitano stated she lost sight of the women, and then observed the front portion of the front loader aggressively shake. Ms. Boitano stated she then observed one of the women walk around from the side of the front loader holding her head. As presented while investigating this crash, the Feeny Brothers work site appeared to be properly set up to mitigate a potential safety incident with ample cones, barriers, caution tape, and signage. There was also a police detail present to assist with pedestrian travel. The area of the work zone where the pedestrans entered from Commonwealth Avenue after crossing Lowell Avenue was taped off with caution tape and a sidewalk closed sign was present. The crash happened inside the work zone parameters.

On August 7th, 2021 at approximately 08:40 hours while working N491, I along with NFD and Fallon ambulance responded to #266 Nevada St for a report of a MV crash involving a pedestrian. On my arrival the fire and medics were speaking with the pedestrian that was struck. He identified himself as SEAN DAZET, DOB 04/3/81, #28 Ashmont Ave, Newton. He stated while walking on the sidewalk with his dog S/B on Nevada St he observed a vehicle backing out a driveway to #266 Nevada St. He said the vehicle was probably ten feet away from him when he yelled out to the driver but the vehicle was already on him. Mr DAZET unable to retreat out of the way put up his left hand to brace for the impact. Mr DAZET was then struck from the vehicles passenger side rear bumper. He was knocked back but never fell to the ground. Mr DAZET reported the vehicle backing out was going he estimated between 5/10 miles per hour prior to the crash. Mr DAZET reported to the medics he was experiencing left hip and left shoulder pain. However he signed a patient refusal. I spoke with the operator of involved vehicle identified as Patrick Alvarez. He stated while he was backing out from the driveway to #266 Nevada St his vehicle made contact with the pedestrian. He further stated he never saw the person walking past the driveway as he was backing up until he heard him yelling out. He immediately stopped his vehicle. The accident was a low impact crash. Mr DAZET was given the operator's information along with his insurance in case he had to seek further treatment for his non visible injuries. Finally Mr DAZET did indicate he probably will follow up with his primary care doctor on Monday. There was no damage to the involved vehicle.

On Wednesday, September 1, 2021, while assigned to Traffic unit N525, I responded to the Newton Centre Municipal Parking Lot (797 Beacon Street, Newton) for a report of a motor vehicle crash involving a pedestrian with a potential head injury. The weather at the time of the crash was cloudy and overcast. The road surface at the time of the crash was dry. The Newton Centre Municipal Parking Lot is open for public travel and use and is maintained by the City of Newton I spoke with the operator of the 2016 Audi 32 (MA: 4PL345) involved in the crash, Mr. Peter Schiller (S26414610). Mr. Schiller stated he entered the Newton Centre lot from Langley Road and made a right turn to enter the back row of parking stalls. Mr. Schiller stated he then took a left turn into the parking area to look for a spot. Mr. Schiller stated he did not see the pedestrian in the roadway in time and his vehicle's front end crashed into the pedestrian. Mr. Schiller estimated the crash happened right near the first car parked in the row closest to where he entered. I showed Mr. Schiller the believed area of impact was actually 3 car stalls in, and the crash occurred in the middle of the travel way. I asked Mr. Schiller if he has any medical conditions or takes any medication that would effect his ability to operator a motor vehicle. Mr. Schiller stated he does not. Mr. Schiller reported no injuries as a result of the crash. I observed very minor damage to the front hood/bumper/fender area of his vehicle from where the crash took place. I did observe soil covering the hood area. The injured pedestrian was carrying a plant with soil in the pot. I asked Mr. Schiller if he could recall where he made contact with the pedestrian or how the pedestrian ended up in front of his vehicle. Mr. Schiller stated he thought the injured pedestrian was running towards his car when the made his turn. I spoke with a witness that remained on scene, Mr. Nicholas Ryland. Mr. Ryland stated he was walking in the Newton Centre lot while pushing his daughter in her stroller. Mr. Ryland stated he observed MV1 enter the lot from Langley Road and took the right turn into the lot at a high rate of speed. Mr. Ryland stated he stopped with the stroller next to the first vehicle parked in the roadway where the last parking stalls begin. The MV1's driver side operated past Mr. Ryland's left side, took a left turn, and then Mr. Ryland stated he heard the crash. The crash took place in the middle of the roadway in between the parking stalls to the East and West of MV1. I responded to Brigham and Women's Hospital in Boston to follow up with the pedestrian injured in the crash, Ms. Sophia Alexandridies (S49399467). Ms. Alexandridies was transported to the emergency department by Fallon Medics with multiple injuries including a head injury. Due to Ms. Alexandridies's injuries, I was unable to obtain a statement from her about the events of the crash. I spoke with Ms. Alexandridies's daughter, Christina, in the emergency department. Christina stated Ms. Alexandridies suffered a head injury, along with possible broken ribs and an injury to her left shoulder. Most of the injuries reported are on Ms. Alexandridies's left side, the presumed area of impact with the vehicle. Due to the circumstances of this crash, and Mr. Schiller being involved in another surchargeable crash on April 28, 2021, a Request for Immediate Threat License Suspension was submitted to the Registry of Motor Vehicles at DCUImmediateThreat@massmail.state.ma.us. Mr. Schiller is also being cited with Massachusetts Uniform Citation 119853AB for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). On September 2, 2021, I contacted Mr. Schiller to provide him with additional information about the crash. Mr. Schiller stated he did not realize Ms. Alexandridies was injuried in the crash. Mr. Schiller also stated he has no recollection of being involved in a crash in April. Mr. Schiller was advised to contact his insurance company to inquire about his last crash. Photos were taken of the area and MV1 and submitted to the IT Bureau. I canvassed the area for survellance footage of the crash with a negative result.

Mv#1 was parked unoccupied opposite #8 Burrage Rd (N/B). #1 was sideswiped by an unknown Mv that left white paint transfer damage on #1's driver's side front end side and also struck the driver's side mirror all causing moderate damage. I spoke to the resident at 8 Burrage Rd. The resident stated a visitor in his house, Jonathan Kaplan, witnessed the accident. The resident stated Kaplan told him he observed a white sedan bearing MA Reg CIAF31 (Mv#2) attempting to parallel park between #1 and another Mv. During the attempt, #2 was observed sideswiping #1. Kaplan further stated a male operator got out of #2, left a note on #1 and drove away. Mv#1 owner/operator Ms Joan Beer stated no note was ever found. Later from HQ, I called the witness phone number provided (617-455-2557) by the resident of 8 Burrage Rd. I called that number, received VM and left a message. A query of #2 registration comes back on a '18 white Lexus G350 to a Mr Barry Benjamin of 79 Florence St #510S Newton. I received information Mr Benjamin's phone number was 339-223-0079. This number when called was not Mr Benjamin's. Later from HQ, I spoke to Ms Beer and provided her with the witness plate number observed as the paint color is a match to the damage sustained on her Mv. She was advised insurance company will investigate further based on current information received. Later, the witness Mr Kaplan returned my phone call and provided the following statement: Kaplan stated he was in his driveway outside directly across the street from where the accident was taking place. Kaplan stated the accident was very obvious as damage was unfolding while Mr Benjamin was attempting to squeeze between Mv#1 and another vehicle. Kaplan stated after realizing he (Mr Benjamin) could not fit he started to drive away. Kaplan stated it drew the attention of another witness (unknown) standing on the street when both walked up to Mr Benjamin's vehicle and asked him if he was going to leave a note for the damaged vehicles. Benjamin refused. Kaplan described Benjamin's overall demeanor as "rude." Kaplan stated he told Benjamin he has cameras capturing the incident. Not until then, Benjamin got out of his car and left a note on only the other vehicle reported to be minor license plate damage. Kaplan stated Benjamin never left a note on Mv#1, the vehicle with obvious moderate damage. Kaplan stated he actually viewed the note left on the other vehicle and said it was illegible. Kaplan told Benjamin this and Benjamin yelled to him "mind your own business" and drove away. Kaplan stated his ring doorbell camera was not positioned to capture any of the incident. Traffic Bureau update (Officer Gaudet): On September 8, 2021 I was able to make contact with the operator of MV2, Mr. Barry Benjamin (S42076881). Mr. Benjamin stated he was operating his 2018 Lexus G350 (MA: CIAF31) at the time of the crash. Mr. Benjamin stated he was backing his vehicle into a parking spot and the space ended up being too small. Mr. Benjamin stated the rear of his vehicle crashed into the front bumper area of MV1, knocking off it's license plate. Mr. Benjamin stated he got out of his vehicle and put the plate back on MV1. Mr. Benjamin stated he left a note in the vehicle with his contact information and left the area. I asked Mr. Benjamin if he was aware of making contact with another vehicle in the area. Mr. Benjamin stated he did not make contact with any other vehicle. Mr. Benjamin stated there was no damage to his vehicle and stated he would e-mail me pictures of his vehicle and pictures of MV1 that he took on scene. Those pictures were forwarded to the IT Bureau to be added to this report. Mr. Benjamin also stated in his e-mail, "My wife informed me that the homeowner who contacted the police called my wifes attention to a car up the street. The car was on the opposite side near a dirt road that had been side swiped earlier. My wife said, "That has nothing to do with us" and the homeowner agreed that that situation did not involve our car." At this time, no charges will be filed against Mr. Benjamin for leaving the scene of property damage.

HE STATED THAT MV 2 WAS PARKED FIVE FEET OFF OF THE CURB IN FRONT OF HIM SO HE HAD TO DRIVE AROUND HIM, AND AS HE WAS DRIVING AROUND HIM, MV 2 BEGAN TO MOVE FORWARD AND STRUCK HIS PASSENGERS SIDE MIRROR CAUSING IT TO SHATTER. ACCORDING TO HERNANNDEZ, MV 2 WAS BEING OPERATED BY A HISPANIC MALE WITH A GOATEE WEARING A HIGH LIGHTER CONSTRUCTION SHIRT AND IN THE PASSENGER SEAT WAS A BLACK MALE WEARING A WHITE T SHIRT AND A BLACK HAT. FERNANDEZ APPROACHED MV 2 IN AN ATTEMPT TO EXCHANGE INFORMATION BUT THE OPERATOR OF MV 2 WOULD NOT. WHEN FERNANDEZ BEGAN WALKING BACK TO HIS CAR THE OCCUPANTS OF MV 2 SWITCHED POSITIONS IN THE VEHICLE AND THE BLACK MALE DROVE OFF TOWARDS WATERTOWN WITHOUT EXCHANGING INFORMATION. MV 2 WAS DESCRIBED AS A BLACK CHEVY FLATBED TRUCK. WATERTOWN WAS NOTIFIED OF THE HIT AND RUN.

At this time MV2 crashed into the rear of MV1. The operator of MV2 states he was traveling east on River Street when MV1 stopped abruptly in front of him and he crashed into MV1. When I arrived on scene both vehicles where facing east on River Street. The keys were in the ignition of MV1 and it was still running. River Street is a public way in the City of Newton. No reported injuries. MV1 was towed from the scene. Citation issued to Operator of MV1 for MGL 90/10 no license.

On 10/7/21 at about 1145am I was dispatched to 1101 Beacon St the Walgreen's parking lot for an MVA with injuries. On arrival, I observed 3 parked unoccupied MVs heavily damaged that were smashed into each other. I was approached by one of the MV owners, Tony Dooley. Mr Dooley stated he was walking out of the Walgreen's front door heading to his parked MV (#3) which was approximately <30 feet away. His attention was suddenly drawn to a speeding MV (MA reg 3MKH84) driving through the lot that veered right and smashed into the 3 cars. Mr Dooley's MV was the middle car smashed between the other 2. The female operator briefly stopped afterwards and pulled into an adjacent parking spot at a crooked angle. I approached the female operator who was being taken out of her car and placed onto the stretcher by medics for transport. Female operator was identified as Ms Norma Greenberg. I asked her what happened? She replied in a bewildered manner "I have no idea." I asked her if she was taking any medications today and she replied yes, to treat a spinal condition she has. Ms Greenberg had lacerations on her legs and arm with heavy bleeding as a result of the impact.Ms Greenberg's vehicle (#1) was towed with significant front damage and airbag deployment. MV#2, the first car struck on impact, was towed with very heavy passenger side, rear end damage and driver's side damage. MV#3, the middle vehicle, was towed with significant passenger side damage, flat tire and driver's side damage. MV#4, sustained significant passenger side damage which was able to be driven from the scene. Based on observations and statements, it appeared Ms Greenberg had suddenly dangerously accelerated through a congested parking lot causing heavy damage. Ms Greenberg's driving history shows surchargable accidents from 7/1/21, 11/1/20, 8/25/15 and 12/26/10. Due to Ms Greenberg's inability to recall how or what happened, I am seeking an Immediate Threat application due to and not limited to medical reasons to suspend Ms Greenberg's license through the RMV.

On 10/19/21 at 08:30 hours, Sgt. Wade notified me that he received an email regarding a motor vehicle accident involving a pedestrian/student that occurred on 10/15/21 between the hours of 08:20am and 08:35am. The crash was never reported by the operator of the vehicle or the student/pedestrian. Approximately two hours later, the student went to the nurse's office seeking assistance for neck pain which is how the school became aware. This crash occurred in the marked crosswalk on Brandeis Road in front of Newton South High School which is located at 139 Brandeis Road. The weather at the time was 63 degrees with mostly cloudy and sunny conditions (wunderground.com). Brandeis Road is a two way roadway divided by an unprotected median that runs east and westbound. In front of the school there is a marked crosswalk in the roadway. There is also a one way driveway near the main entrance of the school with parking stalls and parents can drop their children off there on the right side of the driveway. The driveway then leads back out to Brandeis Road. Right after you exit the lot there is the above mentioned crosswalk to the left of the driveway which is very visible with nothing obstructing your view. On arrival, I spoke to the Dean of the school named Charlie Myette. Mr. Myette brought me to his office and started looking at the school's video surveillance system. He also requested that the student/pedestrian involved to report to his office. The student was identified as Tsukasa Iinishi. I asked Mr. Iinishi what had occurred and he stated that he was walking to school on the morning of 10/15/21. He says that he was wearing a pink hoody sweatshirt with gray shorts. He was walking through the woods which is across the street from the school which exit out to Brandeis Road. He says that he waited at the crosswalk and let two vehicles go westbound by him. He then thought it was clear to proceed and started walking. At the tail end of the crosswalk he was struck by a red colored Toyota Prius which exited left from the driveway and was on the left side of the roadway. He was struck on the left side of his body and he says that he used his left hand to brace himself against the hood and that's when the Prius stopped. He says that the operator of the Prius asked him if he was okay and he replied that he was. He then kept walking toward the school. The operator then continued on it's way without providing any information or notifying us. He described the operator as an Asian female with black hair approximately 40 to 50 yrs in age. During this time, Mr. Myette was able to find surveillance video of the crash. There are two camera angles. One shows the driveway along with the walkway of the school building. The other one shows the same but it also overlooks out in to Brandeis Road. The view of the road is partially blocked from the parked cars in the lot. The video shows a red colored Toyota Prius pull in to the driveway of the school. An Asian female student wearing a white long sleeve shirt with green lettering and dark colored jeans exits the passenger front of the Prius and eventually enters the school. The Prius then pulls out and goes toward the left side of the driveway around other vehicles waiting to exit. At 08:27 hours, the Prius pulls out angling towards the left side of the driveway going around other vehicles waiting to exit. The Prius then exits the lot on the left side (wrong side) of the roadway and makes contact with the pedestrian. In the video you cannot see the front hood area of the car, but you see Iinishi's torso bend towards the vehicle on impact. It shows the Prius stopped there for approximately ten seconds and then it drives away. In the video you are not able to see the registration plate number. Mr. Myette is going to send the photo of the student dropped off in the Prius to his staff to have her identified so I can follow up with the operator. On 10/21/21, I received an email from Mr. Myette stating that some of his staff has not come forward on the identification of the student. He stated that he would now send a message out to a larger group of staff to see if he gets a response. He also attempted to enhance the registration plate using the surveillance software but the plate is still too blurry to make out. On 10/25/21 at approximately 08:40 hours, I went over to Newton South High School to see if I could locate the red Toyota Prius involved in the crash. As I was driving on Olde Field Road approaching Brandeis Road near the school, I observed a red Prius approximately four cars ahead of me in heavy school traffic. Eventually, I observed the Prius pull in to the drop off driveway of the school and drop an Asian female student off. I identified her as the same student that got out of Prius prior to the past accident on 10/15/21. The operator was an Asian female which appeared to be in her late forties. After, the Prius exits the lot on to Brandeis Road and I flagged her down. I had her pull in to the parking lot across the street from the school. I asked her if she was involved in a accident with a student on the morning of 10/15/21 to which she replied that she was. She was identified as Xiaojin Liu through her MA Driver's License # S49033927. She was operating a 2010 Toyota Prius bearing MA reg.# 8JY630. I asked her what occurred and she stated that she was exiting the driveway of the school to turn left on to Brandeis Road. She says that she has a habit to think that no one is ever walking through the crosswalk and thought it was clear to proceed. She turned and then saw the student crossing and attempted to stop but was not able to in time. She then bumped the student with the front of her vehicle. She says that she stopped and asked the student if he was ok several times and he stated that he was. The student then walked away and she says that she did not know what else to do. I advised her that she should of gave her information to the student, notify the school, or call us to which she understood. I issued MA Uniform Citation # 172129AB to Ms. Liu for Ch. 90/24/A Leaving the scene of personal injury. After, I went inside the school and notified Mr. Myette of my findings. I asked him for the surveillance footage as I previously did in person and through email. He stated that he would put the video on a zip drive and notify me.

On 10/21/21 at 15:24 hours, I (Traffic Unit 523) responded to 1479 Washington Street for a motor vehicle accident involving a bicyclist. The weather at the time was seventy three degrees with mostly cloudy skies and some sun(weather.com). The road conditions were dry and Washington Street is a public way maintained by the City of Newton. Washington Street is a two way divided roadway with an unprotected median that runs in a easterly and westerly direction. At this location, there is a marked crosswalk with a walk signal along with traffic signals above in both directions. On arrival, the scene was cleared up and both parties were waiting in the parking lot of the Burke & Blackington Funeral Home at 1479 Washington Street. I spoke to the bicyclist involved who was identified as Rajmattie Thakoordeen. I asked her what occurred and she stated that she was attempting to cross Washington Street in a southerly direction on her electric bicycle. She says that she pushed for the crosswalk signal to activate and once it did she began riding her bicycle when she was suddenly struck by a silver Saab 93. She made it out in to the crosswalk approximately four feet when she was struck by the Saab which was travelling westbound in the right lane of Washington Street. She says that she was struck on the left side of her leg area. She complained of her left leg being sore near her knee area but signed a patient refusal with Fallon Ambulance declining transport. At the time, she was riding a Rad Power Bike Roadrunner 1 Plus that is silver and black in color. It is a Class 2 electric bicycle (750W) that can go up to 20 m.p.h. The only damage it sustained was scrapes to the outside left pedal. Ms. Thakoordeen was wearing black/gray/white pants, yellow reflective jacket, and a black bicycle helmet at the time. Next, I spoke to the operator of the Saab 93 bearing RI registration # FN193. He was identified as Nevan Hanumara with Rhode Island Driver's License # 9960501. He stated that he was travelling westbound on Washington Street in the right travel lane when he came around the curve and noticed the red light signal above in the last second. He says by the time he reacted and attempted to stop he went through the red light and made contact with the bicyclist in the crosswalk. He reported no injuries and no damage to his vehicle. Due to his statement and inattention to the traffic signal, I issued him MA Uniform Citation # 168985AB for Chapter 89/9 Fail to Stop/Yield for the red traffic signal. Digital photos were taken of the scene.

On October 26th 2021 while assigned to the front desk, Hanna Yoon DOB 9/9/83 came in to report a minor past mva/pedestrian. Yoon states that around 0820 hrs today, she was pulling out her driveway at 357 Langley Rd (Ma reg 5HL884). Yoon did not see a pedestrian approach from the right, and contact was made at a slow rate of speed. Yoon states the pedestrian did not fall down. She believes that the front right part of the vehicle hit the person's left thigh. Yoon asked the pedestrian if she was ok, to which the woman stated she was fine. The woman asked for and was provided Yoon's cell phone number before walking away. Her clothing description is unknown. Around 1100hrs Yoon then received a text from 617-913-5141. The person texting identified them self as the woman who was hit, and stated she was in the hospital with leg pain. She then texted that the hospital would contact Yoon for more information. Yoon did not respond and has not been contacted by anyone since this text. A name was not provided by the texter. There are no entries in QED indicating that the unknown pedestrian has called to report this accident. I tried to call the number provided with negative results. At this time, there is no identifiable victim. Yoon was advised that I will document her statement, and in the future it is best to call police to the scene of a crash in order to avoid confusion. Update by Lt. Dangelo: On 10/27/21 at approximately 0830 hrs. the pedestrian from the accident, Woo Jin Seo came into HQ with her mother to report the accident. She stated that on 10/26/21 at approximately 0820 hrs. she was walking to Newton South High School from 383 Langley rd. when a vehicle backing out of a nearby driveway bumped her. She stated that she did not fall down, but did suffer an injury to her leg. She stated that the operator of the vehicle stopped and checked and left her phone number. Ms. Seo went to Beth Israel Lahey urgent care the same day for leg and shoulder pain. I contacted the operator Hanna Yoon and updated her.

Opr. of V1, Juan Chen stated she came out of work and walked to her vehicle. At this time, she noticed a scratch on the left side of the vehicle. Chen stated she thought the vehicle next to her could have hit her vehicle, but did not have proof. The uninvolved vehicle had some damage to the left front. I made contact with the owner and spoke to her. She told me the damage was old from another accident and if she hit the vehicle she would have left a note. Due to the damage being old and the garage not having any cameras, I could not prove who struck V1. V1 sustained minor damage. V1 was not towed and due to no one driving, there were no injuries. Chen requested a report for insurance.

Operator of vehicle one ( 2019 Honda CRV MA reg 2SAH91 ) Elizabeth Sylvia stated that on 11/16/2021 at 16:56 hours she was driving south bound on Needham St near Charlemont St. Needham St is a public way in the City of Newton. Sylvia stated that as her vehicle slowed to turn left onto Charlemont St it's front driver's side struck a pedestrian in a crosswalk. Sylvia stated that she did not see the pedestrian prior to the crash due to the area not being lit. It should be noted that the over head street light was inoperable, and the cross walk was not illuminated or reflective. The crosswalk sign that was posted before the crosswalk was not reflective. Sylvia stated that she was unaware that a crosswalk was at the crash location. Sylvia was not injured in the crash , and her vehicle had no damage to it. The pedestrian Nivicha Morpeau stated that she was in the crosswalk in front of 241 Needham St when she was stuck by the front driver's side of vehicle one. It should be noted that Morpeau was wearing dark clothing at the time of the crash. Morpeau complained of back pain, but refused medical treatment and transportation to the hospital. Morpeau singed a patient refusal form provide to her by Medics. Based upon statements made to me by Sylvia and Morpeau I issued Sylvia MA Uniform Citation T 2016746 and cited her for a violation of Newton City ordnance 19/116 failure to stop for a pedestrian in a crosswalk. I took photos of vehicle one and the disk was forward to the NPD's IT Bureau for downloading.

MV#1 continued to his job site, until he was instructed to the Police Station by his superiors to meet with Ped#1 who was filing a Police Report regarding the collision. -Pas Mv#1statements coincide with those made by OpMv#1. -Ped#1 stated he got out of his vehicle to get the dry cleaning items from the back seat when he was struck by Mv#1's side mirror on his left shoulder. He stated the driver paused and the passenger asked if he was ok. Ped#1 stated "Really, what do you think!?" as he walked toward MV#1. MV#1 then left the scene, Ped#1 stated he contacted the company and reported the incident to them. -PasMV#2 statements coincide with those made by Ped#1. --Ped#1did notreport any injuries at the time of this report and was advised to seek medical attention as needed. There were no vehicles damaged, therefore no vehicle was towed from the scene.

WHEN THE OWNER NOTICED IT WAS HIS VEHICLE HE STOOD BY TO EXCHANGE INFORMATION. OWNER OF VEHICLE #2 STATED HE PARKED HIS CAR IN A STALL AND WALKED AWAY. HE STATED THE CAR IS 17 YEARS OLD AND THE EMERGENCY BRAKE MUST HAVE FAILED AND AS A RESULT HIS VEHICLE ROLLED BACKWARDS OUT OF THE STALL AND HIT VEHICLE #1. VEHICLE #1 HAD RIGHT FRONT QTR. On Wednesday 12/1/21 at approximately 0800 hours while assigned to marked unit n498 I was dispatched to 336 Winchester Street for a report of a single vehicle crash with no injuries. Upon arrival I observed a white BMW X3 partially off the road with heavy front end damage on the Northbound side of Winchester Street in front of 336. I spoke with the operator, PHILLIPS Illana, who states she was travelling Northbound on Winchester Street at approximately 30 MPH when she heard a pop and the car pulled immediately to the right and she felt an impact and the air bags deployed. It appears as though the impact she felt was the Utility Pole in front of 336 Winchester Street. Pole sustained significant damage, but still appeared to be safe. Verizon was notified to respond as soon as possible. Also on scene was STOLYAR Polina, who states that she was walking on the sidewalk toward her house at 345 Winchester Street. Polina states she observed the white BMW travelling at a normal speed Northbound and she also heard a pop and saw a quick flash that she described as a flame come from the passenger side tire of the vehicle causing it to swerve off the road and strike the Utility Pole. Polina stayed on scene to make sure PHILLIPS was ok and to speak with me. After speaking with the operator, PHILLIPS, and the witness, STOLYAR, it appears to me that the passenger side tire of the BMW blew causing the vehicle to veer off the road striking the Utility Pole. I walked Southbound down the Northbound side of Winchester Street to see if I could locate any sort of road defect that could have caused the tire to suffer a catastrophic malfunction. I was unable to locate any such defect. PHILLIPS mentioned to me that recently the light on her dashboard illuminated indicating low tire pressure and she brought the vehicle to have the air pressure refilled. She expressed a concern in hindsight that whomever added air may have over inflated the tire. Exact cause of tire malfunction is unknown at this time.

Vehicle #1 was traveling northbound on Centre St. at approximately 25 MPH when they witnessed someone enter the roadway on foot from between two parked cars and attempted to cross the road. Vehicle #1 stated they attempted to stop but was unable to do so and struck the person with the front left corner and windshield of the vehicle. The pedestrian and owner of Vehicle #2 stated he parked his vehicle on the right side of the road and exited the car. He stated he was walking towards the crosswalk and attempted to cross the street from the front of his vehicle. All he remembers is taking a few steps out into the road, being hit, and flying before hitting the ground. He was unaware where he was struck from. Vehicle #1 had moderate damage to the front and windshield and was towed by Tody's towing. Vehicle #2 was uninvolved in the crash and was parked at the time. The owner of Vehicle #2 and pedestrian struck was injured and transported by Medics to Brigham and Womens Hospital for injuries to his head, left arm, and left leg. Operator of Vehicle #1 stated he was uninjured and refused any medical treatment. It should be noted that the accident happened outside the crosswalk and the street lights above where the accident occurred were not working at the time of the crash making the roadway dark and visibility difficult. Photos of the accident were taken and saved on a memory card.

Operator of vehicle one, Elizabeth Oppenheim stated that while driving her 2021 Subaru Crosstrek SUV ( MA reg 72GL56 ) west bound on Watertown St in front of 392, a pedestrian stepped off the south side walk and into the roadway. Oppenheim stated that she did not see the pedestrian due to low light conditions. Oppenheim estimated that her vehicle's speed at the time of the crash to be 10 to 15 MPH due to heavy traffic. Oppenheim stated that the pedestrian latter identified as Joseph Colucci stepped off the sidewalk and into her path of travel in the roadway. Oppenheim stated Colucci was not in a marked crosswalk. Oppenheim stated that she stopped her vehicle to check on Colucci who told her that he was not injured and did not want medical aid. Colucci stated that he was struck on his right side and fell to the ground. Colucci signed a patient refusal form provided to him by medics. Colucci stated that he did step off the side walk in front of 392 Watertown St to cross the street. Colucci acknowledged that he was not in a marked crosswalk at the time of the crash. Watertown St is a pubic way in then City of Newton.

On 12/13/21 at 11:04 hours, I responded to the Mount Ida Campus of UMASS Amherst located at 777 Dedham Street for a one car crash involving a City of Newton Department of Public Works vehicle. The crash occurred at a y type walkway/driveway near the Shaw Building dining hall. The weather at the time was clear and approximately 50 degrees and the road conditions were dry. On arrival, I spoke to the operator of the DPW vehicle identified as William Bertrand (MA D/L # S24973075 ). Bertrand was operating a 2016 Ford Escape color black bearing MA Municipal registration # M2534A at the time of the crash. He states that he was at the Shaw campus building to conduct an inspection and was parked at a y type walkway/driveway outside of the building. When leaving, he attempted to make a three point turn to exit the driveway. While backing, he states that his foot slipped transitioning from the gas to the brake and he struck a light pole near the edge of the driveway. He says the back up sensors never alerted him of the pole either. The Ford Escape sustained minor scrape marks to the driver's side rear bumper area. As a result of the impact, the light pole cracked at the base and was knocked over on to the ground. Campus Security was on scene and will notify school administration regarding the damage to the pole. I took digital photos of the scene and submitted them to the I.T. Bureau.

Owner of MV1 called in a hit and run. Owner has a dash camera that turns on when the vehicle is hit with force. Owner of MV1 received a notification, walked outside and observed damage to his vehicle. Owner of MV1 showed me the video. I observed the vehicle turn on and a short time later a white Chev pick-up drive by. MV1 suffered damage to the left rear of the vehicle. Due to the low quality picture, I could not see the license plate of MV2. I canvassed the area and found two cameras that were not recording at the time. MV2 did not stop and leave their information. Unable to find an involved vehicle.

On 01/28/2022 at approximately 1245 hours Debra Friedman was walking through the parking lot of Rosenfeld Bagel Co at 1280 Centre St to her car. When she got to her car she was about to open her car door when Vehicle 1, an unidentified black Jeep Grand Cherokee, struck her with the front passenger side bumper and nearly knocked her over. Mrs. Friedman banged on the window of Vehicle 1 in order to alert the driver that they had struck her, however they kept driving and exited the parking lot. Mrs. Friedman described the operator of Vehicle 1 as a white male, possibly in his 60's and possibly wearing glasses. Mrs. Friedman was unable to get the license plate or any other descriptions. Mrs. Friedman stated that Vehicle 1 struck her left side and she was sore around her hip area however did not intend to go to the hospital at the time of this report. On Wednesday, February 23, 2022, while assigned to Traffic unit N525, I responded to the intersection of Washington Street and Elm Street, Newton with N492 for a report of a minor motor vehicle crash. Washington Street and Elm Street are both public ways maintained by the City of Newton. The weather at the time of the crash was clear and sunny. The road surface at the time of the crash was dry. I spoke with the operator of MV1, Mr. Kumar Bhol (S19441804). Mr. Bhol stated his vehicle (2018 Toyota Rav-4 (MA: 8HH159) was parked unoccupied on Elm Street next to the Village Bank in a metered spot. Mr. Bhol stated when he returned to his vehicle after leaving the gym, he observed another vehicle illegally parked behind his vehicle with it's front bumper very close to the rear of his vehicle. Mr. Bhol stated he observed minor fresh damage to the bumper area of his vehicle at this time. Mr. Bhol stated the owner of MV2 returned to her vehicle at this time and he attempted to exchange information with her. Mr. Bhol stated the owner did not want to exchange information and attempted to leave the area. Mr. Bhol stated he positioned himself in front of her vehicle in the roadway as it was leaving to prevent MV2 from leaving. Mr. Bhol stated at this time MV2 made light contact with his knee. Mr. Bhol stated he has no injuries as a result of the contact. Mr. Bhol declined my request to have him evaluated by Newton Medics. Mr. Bhol was also advised to not position himself in front of a moving vehicle to try and stop them from leaving the area. I observed very minor damage to the rear bumper area of MV1. It is inconclusive if the damage is fresh from this crash. I did observe obvious old damage to a sensor on the rear bumper of MV2 with black tape over it. I spoke with the operator of MV2, Ms. Jennifer North (S41819258). Ms. North stated she parked her 2015 Jeep Grand Cherokee (MA: 2XR747) behind MV1 on Elm Street in what she thought was a legal parking space. Ms. North stated when she returned to her vehicle, the owner of MV1 alleged the damage on his vehicle was caused by her. Ms. North stated she did not make contact with MV1 and attempted to leave the area. Ms. North stated Mr. Bhol would not let her leave so they contacted the Newton Police to assist. A query of Ms. North's license status showed it was expired/non renewable. I advised Ms. North of her license status and stated she could not drive her vehicle away from the area. Ms. North stated she understood and walked to her residence with her two dogs. Ms. North's vehicle was left legally parked in a metered parking spot on Elm Street. I observed no noticeable damage to the front bumper area of MV2. Ms. North report no injuries. As a result of Ms. North's license status she was issued Massachusetts Uniform Citation 306986AB in hand for Chapter 90, Section 10 (Unlicensed Operation) and Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care). It is unclear is the damage to MV1's rear bumper was caused by MV2.

Bicycle was traveling either north on Walnut Street or west on Washington Street from the area of Newton Centre. MV1 pulled out from a side street, unknown which side of street, in front of the Bicycle. Causing bicycle and vehicle to collide in an unknown manner. Further causing the bicycle and operator to flip forward in the air. MV1 left the area. MV1 is an unknown description. Operator of bicycle continued on foot on his path home where he was picked up by his father and brought by his father to NWH for cuts on his face and hands. Operator of Bicycle was disoriented and stated he did not know where the accident occurred, manner of collision, MV1's description or if he was wearing a helmet. Operator of bicycle left the bicycle on the sidewalk or in the bushes somewhere on his route home. Bicycle was described as a light blue Trek that was most likely mangled. Bicycle did not have any lights on it. Units canvassed the area and were unable to locate bicycle. Bicyclist was wearing a grey sweatshirt, black jeans and brown shoes. If the Bicyclist and his father find the bicycle later they will call NPD to canvas the area. Traffic Bureau update (Officer Gaudet): On March 1, 2022, I was able to speak with the injured bicyclist, Mr. Yonatan Gershon. Mr. Gershon stated he was traveling on Walnut Street (N) in the right shoulder lane towards the area of Hull Street and Kirkstall Road. Walnut Street, Hull Street, and Kirkstall Roads are all public ways maintained by the City of Newton. Mr. Gershon stated while operating his blue Trek gravel bicycle he continued on Walnut Street when a black vehicle (no further description) appeared out of no where on his left hand side and entered his travel lane. Mr. Gershon stated he did not have time to react and crashed into the rear passenger side of the vehicle. Mr. Gershon stated he does not recall anything between the time he made contact with the vehicle and the time he came to a rest on the roadway. Mr. Gershon stated the vehicle never made an attempt to stop and it left the area. Mr. Gershon stated after the crash he began walking towards the Newtonville Square area until his father picked him up and brought him to Newton Wellesley Hospital. Mr. Gershon stated he suffered injuries to the right side of his face, his right should, right hand, and was diagnosed with a concussion as a result of the crash. Mr. Gershon initially believed the vehicle entered Walnut Street from either Kirkstall Road. Kirkstall Road is on the Eastbound side of Walnut Street and if a vehicle exited the street onto Walnut Street it's driver side would be facing Mr. Gershon's direction of travel. Hull Street is on the Westbound side of Walnut Street. Hull Street supports one-way traffic and traffic can only enter Hull Street from Walnut Street. If the vehicle were to enter Mr. Gershon's travel lane as he described, the vehicle would have to travel Eastbound on Hull Street in violation of the one-way restriction to take a left onto Walnut Street and travel Northbound. Mr. Gershon stated it is possible the vehicle may have exited a driveway from a residence on the Westbound side of Walnut Street, taking a left turn to travel Northbound. After speaking with Mr. Gershon about his injuries and his attempt to avoid crashing into the vehicle, we surmised Mr. Gershon attempted to travel around the vehicle's left side prior to impact, with the right side of his person crashing into the vehicle. Mr. Gershon stated he was not wearing a helmet at the time of the crash and there were not lights or reflective equipment on his bicycle. Mr. Gershon stated he was wearing a gray and white hooded sweatshirt with jeans at the time of the crash. Mr. Gershon stated he returned to the scene after the crash and discovered his bicycle resting near the lawn of a residence on Walnut Street in the area of the crash on the Westbound side of the street. Mr. Gershon stated there was extensive damage to the front wheel and the frame of the bicycle was bent in half. I canvassed the area for black vehicles with fresh damage to their rear passenger side with a negative result. I also canavassed the area for cameras with a negative result. On Tuesday, March 01, 2022 at approximately 1609hrs, I, Officer Fay responded to 1340 Centre St. for a report of a past hit and run. Upon arrival I met with the owner of the vehicles son, William Redmond, who stated on Sunday, February 27, 2022 at approximately 9:30PM he parked his dad's vehicle, a black 2017 Ford Edge (MO plate EEON3z) at the bottom level of the parking garage at 1340 Centre St. William stated that when he returned to use the vehicle on March 01, 2022 at around 3:30PM he noticed a dent in the driver side door. I asked him if anyone else has driven the vehicle within that time, which he replied no and informed me that he did not recall seeing that damage before. Upon closer inspection of the vehicle, I noticed the damage appeared to be cover from dirt and salt from the road, making the damage appear to be old. I also noticed that the parking garage does not have security cameras. At this time it is unknown if the damage is old or if someone had struck his vehicle within the garage between February 27 and March 1st. I informed William that a report would be generated, at which he informed me that he has made his father, Christopher Redmond aware of the damage. The vehicle had minor damage to the door and the vehicle is considered safe to drive and left parked in the parking garage. A vehicle was parked next to William's vehicle, but William stated it was his coworkers and knew the vehicle was not parked next to his vehicle. I walked around his coworkers vehicle, which had no damage.

Operator 1 states he was walking to his vehicle in the parking lot at 999 Watertown St (CVS Pharmacy) when he saw that a vehicle in the parking space to the left of him was too close to his vehicle, preventing him from being able to enter. Operator 1 asked operator 2 if he could back up to create space and when operator 2 reversed, the back passenger side of his vehicle scraped against the left side of vehicle 1's bumper. Operator 1 then asked operator 2 for his registration, operator 2 offered money and then left the parking lot without exchanging information. Upon making contact with operator 2, he stated he was the operator of vehicle 2 and admitted backing up into vehicle 1. Operator 2 stated he was scared to give his information because he does not have a driver's license and he left the area. I observed minor damage to on the left side of the rear bumper on vehicle 1 and minor damage to the right side of the rear bumper of vehicle 2. The operator of MV1 (MA reg 21KM34) stated she parked her vehicle in front of 31 Lincoln Street when her car was hit. The operator came out of Salon Capri and noticed a big dent in the left rear bumper. A witness at the Salon was able to write down the license plate and make of the vehicle that hit MV1. The witness also informed me the car pulled into the Hartford St municipal lot. I walked over to the lot and observed the vehicle was parked and unattended at this time. I attempted to find the owner in the surrounding stores with negative results. I observed the damage on the vehicle and I was able to match it up with MV1. There was damage all along the left passenger side. I left a note on the window of MV2 informing the operator to contact Newton Police The operator of MV2 (MA reg 3VZE99) stated she was attempting to parallel park and did not realize she struck MV1. Both vehicles had minor damage and were able to drive away without any issues.

On April 9th 2022, at approximately 0038hrs, while assigned to cruiser N494, I, Officer Dragone, along with Officer Stake (N491) and Sgt. Brooks (N505) responded to the area of 196 Tremont St (public way in the City of Newton) for a report of a motor vehicle accident. Dispatch reported that the reporting party's vehicle was hit while it was parked in front of his address, and the operator of the other vehicle exchanged partial information and then left the scene. Upon arrival, I observed a blue Honda Accord bearing Massachusetts Registration: 1TDF63 (Vehicle 1) parked on the side of the road, blocking the driveway to 196 Tremont St. The vehicle had heavy damage along the driver's side of the vehicle and back left tire was pushed into the wheel well. In front of 190 Tremont St, I also observed a black BMW 528 bearing Massachusetts Registration: 1FDB38 ( Vehicle 2) with heavy damage along the passenger side of the vehicle and the front right wheel was completely detached from the axel. All the airbags in the vehicle were deployed, and it appeared that vehicle 2 was traveling Eastbound on Tremont St when it crashed into vehicle 1 at an angle. It should be noted that there were multiple deep scratches/dents on the hood, roof, and drivers side panels of vehicle 2 that were not consistent with the crash that just occurred; which indicates the operator was in multiple accidents prior to this one. Pro-tech Towing was also on scene to tow vehicle 2. I then spoke to the owner of vehicle 1, Ryan Desrosiers, who stated that his vehicle was parked on the side of the road when he heard a crash outside at approximately 2345hrs. Mr. Desrosiers stated when he went outside, he discovered his vehicle pushed in front of his driveway, approximately 10-15 feet from where he parked it, and the operator of vehicle 2 standing outside. Mr. Desrosiers stated when he spoke to the operator, he smelled like alcohol and had slurred speech. The operator, later identified as, Benjamin Wadsworth, told Mr. Desrosiers that he was "coming from in town" and "probably should have taken a Lyft tonight." When Mr. Desrosiers tried to exchange information, Mr. Wadsworth took Mr. Desrosiers' phone number and then texted him a link to a generic RMV website. Mr. Desrosiers then said Mr. Wadsworth called a tow company for his vehicle and insisted on paying out of pocket tomorrow for the damage to Mr. Desrosiers' vehicle but told him if he wanted he could call his insurance company. According to Mr. Desrosiers, Mr. Wadsworth then walked Eastbound on Tremont St towards Brighton. It should be noted that Mr. Wadsworth did not provide his name, address, or insurance company to Mr. Desrosiers prior to leaving. I had dispatch run a query on vehicle 2 (MA Reg: 1FDB38) and it came back registered to Benjamin Wadsworth out of 13 Charmane Rd East Falmouth, Massachusetts. While speaking with Mr. Desrosiers, the driver of the tow truck called Mr. Wadsworth cell phone to notify him that he was not going to be towing his vehicle. I was then able to get on the phone and speak with him, and he identified himself as Benjamin Wadsworth and admitted to being involved in the accident that just occurred. When I asked Mr. Wadsworth his location because I was concerned for his well-being due to the severity of the crash and airbag deployment, he stated he was at his apartment in Brighton. When I asked for the address of his apartment, he refused to give it to me and stated he was fine. I stated to Mr. Wadsworth that because he only gave limited information and then left the area that he could be charged with Leaving the scene of a collision, to which he replied, "I am in law school you can see the books in the back of my car, you can't do that, you have my plate, he can look that information online." When asked about having an address out of Falmouth, Mr. Wadsworth stated he has two addresses because he attends school in the area. It should be noted that while speaking with Mr. Wadsworth on the phone, his speech was slurred, slow, and dilerbrate, consistent with being intoxicated. Tody's towing arrived on scene and took possession of Mr. Wadsworth's vehicle. I then tried a second time to speak with Mr. Wadsworth via a phone call to get him to agree to a well-being check. Mr. Wadsworth, again, refused to provide me with his address in Brighton, insisting he was fine. Although Mr. Wadsworth sounded intoxicated, he did not sound injured and was coherent when answering my questions. Due to Benjamin Wadsworth operating recklessly, knowingly getting into a collision causing property damage, not providing his name, not providing his address, and leaving the scene, I am issuing him Massachusetts Uniformed Citation T1445620 for the following: MGL. Ch. 90 S.24 (Leaving the scene of a collision after property damage) MGL. Ch. 90 S.24/E (Negligent operation of a motor vehicle) A Request for Immediate Threat License Suspension/Revocation form was filled out and faxed over to the RMV.

At approximately 1840HRs on Wednesday, June 08, 2022, I was dispatched to 125 Parker St. for a road rage incident. Upon arrival, I spoke with Derek Eggleton, who stated that he was walking from his truck on Glenwood Ave. to 125 Parker St. with building supplies in hand. As he crossed Parker St., he noticed a Maserati bearing MA Plate: SUGAR approaching him, southbound, at a high rate of speed. Derek then threw said building supplies and moved out of the way of the vehicle because he believed it was not going to stop. He then approached the vehicle which had slowed down and Derek states it "tapped" him in the leg. Heated words were exchanged between Derek and the operator of the vehicle, identified as Sidney Gersh, who ultimately drove off. While I was on scene, Officer Hatfield spoke with Sidney, who stated we was indeed involved in the incident, but states he did not hit Derek. Officer Hatfield observed no damage to the vehicle and I observed no marks on Derek, who signed a patient refusal. I advised Derek that I would be documenting this incident and explained to him that if he so chooses, he may seek charges through Newton District Court. He stated that he understood and would take that into consideration.

The operator of MV1 stated he was traveling on North Street, driving approximately 70 mph and a skunk ran across the street. The operator stated he swerved to avoid the skunk, hit the curb and lost control of the vehicle. He stated once the car rested against the garage to 105 North Street, he and the passenger got out of the vehicle. He stated the passenger told him to leave. The operator stated he walked down Farwell Street and after thinking about what happened he came back to the scene of the accident. The operator and passenger stated they were not injured. After speaking with the operator, I observed tire marks going South on North Street and followed them to the curb where I believe he struck which was located approximately 100 yards, in front of 129 North Street. From my observations, MV1 struck the curb located in front of 129 North Street. After striking the curb, MV1 spun sideways, continued traveling southbound, until the MV crossed into southbound lane, struck the curb and then the utility pole 14. After striking the curb the MV spun around facing north, struck the retaining wall, bushes and light pole, located in front of 107 North Street. After striking the light pole, the vehicle hit the garage door at 107 and 105 North Street, resting against 105 North Street. There was significant damage to the garage doors to 105 and 107 and damage to the support beam located between the residences. MV1 sustained major damage to the motor vehicle. Todys was notified for a tow. Photos were taken of all damage and submitted to IT

MV 1 was traveling straight northbound on Cherry St when he rear ended MV 2. The operator of MV 1 (Mr. Richard Antonellis) stated he did not see the parked vehicle due to poor lighting in the roadway. I observed the street light where the crash occurred to be blocked by multiple trees on the side walk. The operator of MV 1 was not injured during the accident and signed a patient refusal with Fallon medics. The passenger of MV 1 sustained a minor cut on her nose and was treated on scene with Fallon medics and signed a refusal to be transported. MV 1 sustained major damage to its front and right side. Front and side airbags also deployed. MV 1 was towed by Tody's due to it being disabled MV 2 was legally parked on the right side of Cherry St facing Northbound. MV 1 rear ended MV 2. MV 2 was unoccupied and parked when the crash occurred. MV 2 sustained major damage to its rear and left side. MV 2 was towed by Tody's due to it being disabled.

On Tuesday, June 28, 2022, while assigned to unit N496, I responded to the area of Langley Road and Hamlet Street, Newton for a report of a roll over crash with 3 injured parties. The weather at the time of this crash was clear and sunny. The road surface in the area of the crash was dry. Langley Road and Hamlet Street are both public ways maintained by the City of Newton. I approached the area of the crash coming from Boylston Street and when I arrived my attention was drawn to two locations on Langley Road. I observed a female party laying prone on the sidewalk area of Langley Road (E) at the Hamlet Street intersection. This female party, later identified as Ms. Melinda Conroy, was being attended to by passerby's in the area. My attention was then drawn to an overturned vehicle resting half in the roadway/half on the sidewalk (W) in front of 323 Langley Road. As I began to run over to the vehicle, I observed a woman standing next to the vehicle screaming frantically. This party identified herself as the driver of the lone vehicle involved, Ms. Natalya Kabanovsky (S65512051). There were parties at the back of the vehicle yelling to me that there was a woman trapped under the vehicle and that they were holding the vehicle up to get her out. I ran over to the rear of the vehicle and observed a female party, later identified as Ms. Annette Sooper, lying underneath the vehicle in the roadway. There was a large pool of blood on the roadway as the result of a laceration to what appeared to be her head/face area. Due to the unknown stability of the vehicle resting on its roof, and due to the passerby's stating they were holding the vehicle up so it did not roll over onto Ms. Sooper, I reached under the vehicle from the rear, grabbed Ms. Sooper's person, and dragged her away from the vehicle to a safe location on the sidewalk. At this time, I observed a large laceration to Ms. Sooper's head and she stated she had suffered a leg injury. Newton Fire and Medics along with additional Police units arrived on scene and took over rendering aid to the victims. I then started to conduct my investigation into the crash. I spoke with the operator of the vehicle, Ms. Kabanovsky. Ms. Kabanovsky stated she was operating her 2005 Lexus ES330 (MA: 293TAB) on Hamlet Street (W) towards Langley Road. Ms. Kabanovsky stated as she approached the intersection, a female party (Ms. Conroy) ahead of her was running across Langley Road (S) at Hamlet Street. Ms. Kabanovsky stated she attempted to hit her brakes, but instead hit her gas pedal. Ms. Kabanovsky's vehicle crashed into Ms. Conroy and then she panicked. Ms. Kabanovsky stated her car then accelerated across the street and crashed. Ms. Kabanovsky stated she was not wearing a seatbelt at the time of the crash and extricated herself from her vehicle after it rolled over onto its roof. I requested Ms. Kabonovsky's license while speaking to her, but she was too upset to located it. I queried Ms. Kabanovsky's license and registration status through Newton Dispatch. Ms. Kabanovsky's license stated is expired as of 12/18/2016. Ms. Kabanovsky registration is active. I observed substantial damage to the entire front end, driver, and passenger side of Ms. Kabanovsky vehicle. Ms. Kabanovsky was evaluated by Newton Medics and complained of right arm pain. Ms. Kabanovsky was also visibly distraught and expressed concern over the two pedestrians involved in the crash. Ms. Kabanovsky was transported to Brigham and Women's Hospital for treatment. I briefly spoke with Ms. Melinda Conroy as she was being attended to by Newton Medics and Fire. Ms. Conroy stated she was running on Langley Road (S) and observed MV1 traveling on Hamlet Street (W) towards Langley Road. Ms. Conroy stated she believed MV1 was slowing down and coming to a stop. Ms. Conroy stated she waved at the vehicle and as she crossed the intersection, MV1 accelerated and crashed into her left side. Ms. Conroy stated she flipped over the vehicle and landed on the roadway. Ms. Conroy was conscious and alert when speaking to Newton Medics and was transported to Beth Israel Deaconess Hospital in Boston. I later spoke with Ms. Conroy after her release from the hospital. Ms. Conroy stated she suffered some bumps and bruises had no major injuries. Officer Pohlman spoke with Ms. Sooper prior to her transport to Beth Israel Deaconess Hospital in Boston by Newton Medics. Ms. Sooper stated she was walking on the sidewalk in front of 323 Langley Road after returning from Newton Centre. Ms. Sooper stated as she was walking, she observed MV1 hit Ms. Conroy and then travel across the street in front of her into a tree. Ms. Sooper stated she does not remember what happened after that. I spoke with Ms. Sooper's son at NPD HQ and was advised Ms. Sooper was admitted to the hospital and suffered a fracture of her left hip and required 6 staples to close a laceration on her head. Tody's Towing responded to the scene and removed MV1 from the roadway. Traffic Officer Wilson took pictures of the scene and submitted them to the IT Bureau. After speaking with all parties involved, Ms. Kabanovsky will be cited with Massachusetts Uniform Citation 494688AB in hand at her residence for Chapter 90, Section 10 (Unlicensed Operation), Chapter 90, Section 24 (Operate To Endanger), and Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Ms Kabanovsky was also cited with Massachusetts Uniform Citation 494800AB in hand at her residence for Chapter 90, Section 13A (Fail To Wear A Seat Belt). A Request for Immediate Threat License Suspension/Revocation was completed and submitted to the Registry of Motor Vehicles with supporting documentation via e-mail, DCUImmediateThreat@massmail.state.ma.us.

Narrative:
On 07/07/22 at 11:44 hours, I (Traffic Unit # 523) responded to 68 Wyman Street Apt. # 5 for a past motor vehicle crash involving a pedestrian that occurred on 06/09/22 at 808 Beacon Street. This area of Beacon Street is a public way maintained and owned by the City of Newton. The weather at the time was 81 degrees and partly cloudy (wunderground.com). The road conditions at the time were dry. On arrival, I spoke to the pedestrian involved in the crash identified as Thomas Jones. He stated the crash took place on 06/09/22 approximately between the hours of 14:05 and 14:10. He further stated that this occurred at 808 Beacon Street while he was crossing Beacon Street near the Bank of America. He says that he pushed the button to activate the crosswalk yellow flashing signal lights. It should be noted that I am familiar with this location and it does have a marked crosswalk on both sides of 808 Beacon Street with yellow flashing signals that can be activated. Once activated, he says a vehicle traveling eastbound on Beacon Street stopped for him. He then entered the crosswalk from the eastbound side of Beacon Street going across the street with his walking cane. He kept walking in to the westbound travel lane of Beacon Street when he was suddenly struck by a motor vehicle. Thomas could not provide what type or color of the vehicle. As a result, Thomas fell forward and was knocked down to the ground on both hands and knees. He also says that his right cheek and eyeglasses struck the pavement which damaged the lens to his glasses. He sustained a bruised (black and blue) right eye. He says approximately seven days later he started having neck pain to which he has been receiving medical attention for. He also states an upper right side tooth implant fell out of his mouth. After the crash, he states he was helped up by two unknown bystanders. An elderly white female who was operating the vehicle that struck him approached him. Thomas says that the female told him that she did not see him crossing the street as she was looking down and not up at the time. She provided him with her name of Jill Levin and her telephone number before leaving. Thomas states he was wearing gray colored khaki pants, sneakers, and a light blue t-shirt at the time of the crash. Next, I attempted to call the number for Jill Levin and left her a message to call me back. At approximately 14:45 hours on this date, Ms. Levin came in to NPD Headquarters and spoke to me. I asked her if she was involved in a accident during the month of June. She stated that there was an incident on a Thursday approximately three weeks ago in June of 2022 at approximately 14:30 hours. She says she was driving her 2013 Lexus 450RX color gray bearing MA reg. # 6RK464 westbound on Beacon Street near address number 808. She saw an older male crossing in the crosswalk and she says that she stopped for him. The male then suddenly fell down in front of her car. She states that she never struck him with her vehicle. She then got of her vehicle and asked him if he needed any assistance to which he replied that he was fine and did not need anything. He provided his name to her. She then provided her name and phone number on a piece of paper. I asked Ms. Levin why she would provide her information to Mr. Jones if no accident occurred. She replied that it was the right thing to do as far as checking on him and seeing if he needed any assistance. I then inquired why Mr. Jones would alledge that she stated to him on scene that you did not see him crossing the street. Also, that you were looking down and not up. Ms. Levin denied ever stating this to Jones. I then went outside and looked at her Lexus 450RX to check for any damage to the front end of the vehicle. I observed two small scratches to the passenger side front bumper which appear unrelated. The marks were indicative of white paint transfer from having contact with another vehicle. Ms. Levin did not know where the marks came from. I advised her that I would investigate this further and check to see if there is surveillance footage from the Bank of America or other source. After, I attempted to call Bank of America and could not reach anyone there. I will check the area and go to the bank on 07/08/22 during my next shift. On 07/08/22 at approximately 09:10 hours, I went up to 808 Beacon Street and observed no other surveillance cameras in the area. I went in to the Bank of America and spoke to the branch manager. He showed me the multiple camera angles they have, but none show out in to Beacon Street. As a result of this investigation and the statements made by both parties, I believe that Mr. Jones was struck by Ms. Levin's vehicle. The fact that Ms. Levin got out of her vehicle to check on Jones and provide her information to him even though she claims no accident took place is highly unlikely. Therefore, I am issuing Ms. Levin MA Uniform Citation # 507419AB for Chapter 89/11 Crosswalk Violation. The citation was mailed out on 07/08/22. I spoke to Ms. Levin on the phone explaining a citation was issued and mailed to her.
