On Wednesday, January 9, 2019, while assigned to Traffic unit N525, I responded to 457 Walnut Street, Newton for a report of a past motor vehicle/pedestrian accident involving a student at Newton North High School. I met with the pedestrian involved in the accident, Ellie McBride, in the Riley House admin office. McBride is a Freshman at Newton North High School. McBride's mother, Linda Hanusaik, was also present while I spoke with her. McBride stated at approximately 0740 hours, she was attempting to cross Lowell Avenue in a marked crosswalk at Highland Street (E). McBride stated she observed a newer model gray Acura SUV traveling towards her Southbound on Lowell Avenue while she was standing in the crosswalk. McBride stated she thought the vehicle was going to slow down for her. McBride stated as the vehicle got closer it appeared to be slowing down but was unable to avoid making contact with her. The front end of MV1 made contact with McBride's left side. The impact caused McBride to fall down onto the roadway. McBride stated the vehicle pulled over and the operator exited the vehicle to check on her. McBride stated the operator was a white female with long brown hair, possibly high school aged. McBride stated after she told the operator that she wasn't hurt, the operator returned to her vehicle and left the area, possibly towards Commonwealth Avenue. McBride could not provide any additional information about the operator of the vehicle involved. McBride stated she then walked to Newton North High School. McBride stated she went to her morning class but left and texted her mother to tell her she was involved in an accident. McBride stated she was shaken up by the accident, but reported no injuries. Newton Medics evaluated McBride and obtained a patient refusal. McBride was advised that due to the reported age of operator, they may be a student at the high school. McBride was advised that if she sees the operator to obtain her information/contact NPD. I checked the area of Lowell Avenue and Highland Street for the vehicle involved/witnesses with a negative result. A portion of Newton North's athletic fields are next to this intersection. Newton North stated there are no surveillance cameras in this area. Lowell Avenue and Highland Street are both public ways maintained by the City of Newton.

VEHICLE #1 STATES SHE HAD NO INJURIES. ADVISED TO CONTACT HER INSURANCE COMPANY. ON 1-24-19 AT APPROX. 0930HRS. WHILE WORKING N492 I WAS INFORMED BY CIVILIAN EMPLOYEE ( STACY ANNESE ) THAT THE OWNER OF VEHICLE #1 ( AYENG FONG ) HAD COME TO THE STATION TO INFORM ME THAT THE ORIGINAL LATE NUMBER SHE HAD GIVEN ME WAS WRONG. SHE STATE THE PLATE NUMBER OF THE VEHICLE THAT ALLEGEDLY HIT HER CAR WAS ( MA REG: 9TK518 ). A REGISTRY CHECK CAME BACK TO A 2019 BLACK JEEP CHEROKEE OWNED BY CHRISTINE MITCHELL OF 169 ADAMS ST. I WAS UNABLE TO MAKE CONTACT WITH MS MITCHELL AND WILL ATTEMPT O DO SO ON NEXT SHIFT. Traffic Bureau update (Officer Gaudet): I attempted to contact the owner of MV2, Anapaula Guedes (S28004541) with a negative result. A Hit and Run inquiry was mailed to the Guedes.

On the above date/time I responded to Lexington Street / Freeman Street for a two motor vehicle crash with air bag deployment. Upon arrival on scene I checked all parties for injuries. Operator of vehicle one Ms. Farnese complained of head / neck injuries and Fire and Medics responded on scene. All parties in vehicle two did not have any injuries. I spoke with the operator of vehicle one Ms. Farnese in the ambulance she stated she was slowing traveling south on Lexington Street at the intersection of Freeman Street due to the traffic signal for Lexington Street was yellow. She stated as she was slowing she felt a heavy impact from in the rear by vehicle two. I spoke to the operator of two Ms. Heimbrock, she stated she is a pastor for the Salvation Army in New York and she was transporting two children Shamah Lapaix d.o.b. 4-18-14 and Grace Lapaix d.o.b. 1-10-11 to a Christian school in Wayland. She stated that she is staying with the children at 65 Falmouth Road Newton MA with the childrens grandmother due to the fact the parents are in New York doing a continuing education program until Friday January 18, 2019. She stated she is not familiar with the area, she was traveling south on Lexington Street approaching Freeman Street when she glanced at the map on her cell phone. She stated she did not observe the color of the traffic signal and did not observe the rear brake lights in vehicle one. All parties were evaluated in vehilce two by medics with no injury. I spoke with the childrens mother Sarah Lapaix 1-203-685-2809 who confirmed Ms. Heimbrock's statement about the care of the children. I did observe an expired inspection sticker as of 12/18 on vehicle one. Therefore due to operator of vehicle two's statement I checked the rear brake lights. When the brake pedal was pressed the left brake light and top(3rd) brake light was operational but the right rear brake light was not operational. Tody's towing arrived on scene and towed both vehicles top their lot. Ms. Heimbrock made arrangements for transport of herself and the children. Ms. Farnese was transported to Newton Wellesley Hospital for non life threatening injuries. I advised all parties of the process and gave them each others information. I checked the traffic signals in the intersection and they were fully functional. Therefore with all the above mentioned I am issuing citations to both parties Operator of vehicle one citation # T1440538 for Chapter 90 Section 20 EXPIRED INSPECTION STICKER $ 55.00 Operator of vehicle two citation # T1440539 for City Ordinance 19-75 FAILURE TO USE CARE IN STOPPING $ 20.00 Both citations were filled out after investigation and will be mailed to their license address.

On Wednesday, January 16th 2019, at approximately 3:30pm, I, Officer Brooks, responded to a single car MVA in the area of 611 Watertown street. Upon my arrival Cataldo medics were treating the operator of MV1(MA REG 159EF6) for a diabetic issue. I spoke with a witness to the accident who did not wish to leave their information. The witness stated they were traveling westbound on Watertown street behind MV1, when it suddenly left the roadway to the right, striking the curb, before returning to the roadway and stopping. The witness stated that MV1 nearly missed a fire hydrant and a utility pole on the sidewalk. MV1 had a flat front passenger side tire, and minor scrapes on the body. I spoke with the medics who were still treating the operator for her diabetic issue. I observed the operator was in and out of consciousness, and unable to speak about the accident. It took several minutes for the operator to regain her consciousness, however she was still not able to answer simple questions coherently and was transported to Newton Wellesley hospital for further evaluation. MV1 was towed from the scene by Tody's, and an inventory form was filed and submitted. The area the crash occured was a high pedestrian traffic area, and both an elementary and middle school are within walking distance of the accident. An immediate threat form was filled out and filed for the operator of MV1.

MA commercial reg M94754 (vehicle #1), a City of Newton pickup/sander truck, was located crashed over the curb in front of 1970 Beacon St. The vehicle was unoccupied, had the keys in the ignition, and was reported stolen just a few minutes prior to discovery. The reporting party of the stolen truck and uninvolved occupant is identified as City DPW worker George Sementelli. The witness/passerby who discovered the collision is also a DPW worker, identified as Donny Brandt. Pursuant to an investigation outlined in incident #19002213, the operator at the time of the crash was determined to be Michael Carter. It is believed that Carter stole the vehicle from 295 Auburn St, fled the area, and ultimately crashed in front of 1970 Beacon St. Corroborating facts outlined in the incident report put Carter as the operator. The vehicle itself was located partially on the sidewalk and partially on the road, therefore Carter is being cited with MGL 89/4A - Marked Lanes Violation (T1268423). Due to the fact it is believed that Carter fled the scene after the crash, MGL 90/24/C - Leaving the Scene of Property Damage was added as well as MGL 90/23/D Operating After a Suspended LIcense, and MGL 90/24/O - Reckless Operation of a Motor Vehicle. The crash occurred during busy weekday commuting about 0.7 miles from Angier Elementary School. The last 3 charges mentioned have been listed on a second citation (T1268422). Officer Brooks also issued T0648082 for OUI Drugs MGL 90/24/G. See the incident report for details about this charge. In total, there are (3) citations issued to Carter. The vehicle sustained heavy front end damage. It should be noted that the utility pole a couple of houses down from the crash site (Beacon/Varick Rd, pole #212) sustained fresh, minor superficial damage, consistent with the vehicle crashing into it before verring onto the sidewalk. Pictures have been taken and submitted to the IT bureau. Tody's responded and towed the vehicle to the police garage for processing due to the pending other criminal charges involved in this accident/incident. Michael Carter was subsequently placed under arrest and his citations have been placed into his property. Carter suffered a hand injury due to the crash an was transported to NWH Hospital for an evaluation after he was booked.

The operator of MV#1 stated she was travelling southbound on Walnut St at the intersection of Elm Rd when she let a vehicle out of Elm Rd to make a left turn. The operator of MV#1 stated she proceeded to move forward when a school bus directly behind that vehicle also made a left turn making contact with MV#1. The operator of MV#1 honked her horn to get the school bus's attention with negative results. MV#1 sustained minor damages to its driver's side front bumper and fog light. The damage to the school bus should be to its driver's side rear bumper. There were no reported injuries to the operator of MV#1. There were no reports of any contracted school bus operators reporting an accident. The operator of MV#1 stated there were witnesses at the accident scene but she did not get any of their information. The operator of MV#1 was advised to contact her insurance provider.

MV1 had just picked up a student at 56 Crescent Ave(at the intersection of Throwbridge). He made the pick up just west of the stop sign. He turned off his illuminated bus stop sign, paused and then began to pull into traffic. MV2 was stopped at the stop sign on Trowbridge st at Crescent St waiting for the bus to turn off the stop sign. When the bus turned it off, it appeared to him that the bus was not pulling into traffic. MV2 began to turn left "very slowly". As he was driving next to the van, the van pulled into traffic and the 2 MVs touched mirrors. There was no damage to either MV. A BLS ambulance was called to the scene because there were three students on board. All parents showed up and signed refusals for their children. Pictures were taken of the two mirrors that had no damage. The following parents showed up on scene to sign refusals for their children: Gareth Gorman for Sophia 617-515-1576 Lori for Harry Melchionno 617-527-7751 Malagros Falu for Nathalie Cedno The students belong to the Oak Hill Middle school and they were notified.

On the above date/time I responded to the area of Cherry Street and Webster Street for a three motor vehicle crash with injuries. Upon arrival along with Sgt. Dempsey, Fire/medics. I spoke with the operator of vehicle one. He stated he was stopped on Webster Street facing westbound a the red traffic signal. He further stated the traffic signal on Webster Street turned green, he accelerated forward. He stated vehicle two traveled north on Cherry Street at a high rate of speed through the red traffic signal. He stated he had no reaction time due to the speed of vehicle two and had impact. I spoke to the operator of vehicle two. He stated he was traveling north on Cherry Street going too fast, he stated he thought it was a school zone so he slowed down and he did not observe the color of the traffic signal. He stated he observed vehicle one come out of the Webster Street at the last second at impact. I spoke with the operator of vehicle three. She stated only remembered being stopped on Cherry Street facing south. She stated she did not remember what color the traffic signal was, just knew she was stopped when she felt heavy impact from vehicle two. Injuries: Oper of V1 none at this time- patient refusal Oper of V2 none at this time - patient refusal Oper of V3 Head laceration and head trauma, transported to Newton- Wellesley Hospital Tody's towing arrived on scene and towed vehicles two, three to their tow lot. All proper paper work was done for a towed vehicle(s). All parties advised of the process. After the vehicles were towed, and the scene was cleared I watched the cycling of the traffic signals in the intersection and observed they were functioning properly. It appears a this time based on statements and physical characteristics of the impact area on the roadway, that vehicle two went through the red light on Cherry Street, which caused the crash.

MV1, a City of Newton contracted school van, was the first car stopped at the red light north on Walnut St at Homer St. MV2 was slowing as she approached the red light and rear ended MV1. According to MV1, driver of MV2 got out of her MV and looked at both MVs. She then got back into her car and when the light turned green they both pulled over just past the intersection. MV2 asked MV1 if she hit him and he stated yes. She stated that she did not see his van, which is bright blue. MV2 operator stated to me that she was stopped at the traffic light and thought MV1 backed into her. When I told her MV1 statement she didn't argue. There was minimal damage to MV1. The outline of MV2 license plate was imprinted in dust on the bumper. There was no visible damage to MV2. I took pictures of the damage to MV1 and MV2. The following parents responded to sign refusals for their children: Wenlei Zhou for Sohia Zhou Katie Thatcher for Tristan Thatcher Both students went to school. The students go to the Newton Early Childhood school on Jackson Rd.

MV1 a City of Newton school bus was stopped southbound on Parker St at Rte 9 east to turn left onto the ramp. MV2 began passing the bus on the right when the bus turned left onto the ramp. The right rear corner of the bus struck the front left wheel well of MV2. I took pictures of the damage to MV2 and TOT IT. I spoke with Regina Moody of transportation and she put me in contact with the driver of a bus who was in the area at the time of the MVA. On Thursday I met with Joseph Montuna who was in the area at the time of the MVA. His bus is 362. The side of his bus says Eastern Bus Company. It does not say Newton Public Schools anywhere visible. I looked at the right rear of the bus that would have come into contact with the MV. There was not even a smudge on the bumper. I am not positive that this is the bus involved. I have attempted to contact MV2 for clarification to positively ID the correct bus. The number provided goes directly to VM which is full. It is possible that this was a non Newton school bus involved that I am unable to identify.

Narrative:
On Wednesday, April 10, 2019 while assigned to Traffic unit N525, I responded to the intersection of Centre Street and Wesley Street, Newton for a report of a motor vehicle accident involving a bicycle. It was also reported that the parties involved were arguing with each other. The weather at the time of the accident was cloudy and the road surface was dry. Centre Street and Wesley Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Abdulaziz Stewart (S04075169). Stewart stated he was operating his 2017 Toyota Camry (MA: 669FM3) at the time of the accident. Stewart stated he came to a stop on Wesley Street (E) at Centre Street and was attempting to take a left turn onto Centre Street. Stewart stated there were no vehicles in the Southbound lane of Centre Street and there was a vehicle stopped in the Northbound lane of Centre Street motioning to him to take his left turn. Stewart stated as he began to start his left turn, a bicyclist crashed into the front passenger area of his vehicle. Stewart stated the bicycle had been traveling Northbound on Centre Street in the Southbound Lane. Stewart stated after the collision the bicyclist was yelling at him and called him a "Nigger". Stewart and his occupant, Miriam Stewart, reported no injuries. Miriam is a student at Bigelow Middle School. I observed minor damage to the front passenger quarter panel area of MV1. I spoke with the bicyclist involved in the accident, Timothy Murtagh (S49233464). Murtagh stated he was traveling Northbound on Centre Street on his Cannondale bicycle in the Southbound lane. Murtagh was not wearing any safety equipment while operating his bicycle (helmet) and was wearing a dark red jacket. Murtagh stated as he approached Wesley Street, MV1 "jammed on the gas" and hit him in the roadway. Murtagh stated he then fell to the ground and landed in the middle of the street. While speaking with Murtagh I told him it was reported to me by the operator of MV1 that he used a racial slur during their interaction. Murtagh acknowledged that he did and stated, "I called him a nigger". I observed moderate damage to the front wheel/rim of Murtagh's bicycle. Murtagh reported no injuries. Murtagh was advised and issued Massachusetts Uniform Citation T1443441 for Chapter 85, Section 11B (Fail to ride on the correct side of the roadway). Photos were taken at the scene and submitted to the IT Bureau. Due to the language used by Murtagh during his exchange with Stewart, Officer Rosenbaum of the Community Services Bureau was made aware of the incident for follow up.

On Wednesday April 10th, 2019 at approximately 0755 hours I responded to the area of Centre Street and Washington Street for two car motor vehicle accident involving a Newton school bus with children on board. Centre Street and Washington Street are both public ways in the City of Newton. At the time of the accident the weather was cloudy with light rain. The road conditions were fairly dry. I spoke to the operator of MV1 identified as Julie Lym (S16060051) who stated she was traveling northbound on Washington Street onto the Centre Street bridge. MV1 stated she was attempting to cut across lanes to merge on to the Mass Pike Westbound. MV1 stated traffic was at a standstill and she saw an opening to change lanes. MV1 stated MV2 was stopped in front of her and as she attempted to change lanes her vehicle side swiped the rear drivers side corner of the school bus. The operator of MV1 reported no injuries. MV1 sustained minor damage and was able to leave the scene. I spoke with the operator of MV2 identified as Antonio Mendes Goncalves (S54275868) who stated he was stopped at the red light in the right middle lane on the Centre Street bridge. MV2 stated MV1 attempted to change lanes and struck the rear drivers side corner of the school bus. MV2 sustained minor damage and the operator reported no injuries. MV2 is a City of Newton contracted yellow school bus that had six children present on the bus at the time of the accident. The school bus was transporting th e children to Bigelow Middle School. All children present reported no injuries and their parents were notified by the school administration. I escorted the school bus back to the school where the children were all evaluated by the school nurse. 14 photographs were taken and submitted to the IT Bureau.

Mrs. Weiss stated that she was attempting to back out of her driveway at 285 Crafts Street when her vehicle accelerated for no reason "like a bat out of hell." The vehicle then crossed Crafts street at a high rate of speed, jumped the curb, crossed the large grass island, and only stopped when the vehicle crashed into a tree. Mrs. Weiss was uninjured and signed a patient refusal. The vehicle sustained moderate damage to the tires, undercarriage, and rear bumper and was towed from the scene by Todys. Photos were taken of the damaged city tree and were placed in the IT bureaus mailbox to be attached to this report. Based on the facts that Mrs. Weiss could not explain the reason for the cars acceleration, the distance the vehicle traveled without stopping, and the close proximity of the crash to an elementary and middle school I filed a request for Immediate Threat License Suspension/Revocation. All proper paperwork as well as a copy of this report were faxed to the Registry of Motor Vehicles.

On May 3, 2019 at approximately 18:18 I responded to a motor vehicle crash on Beacon St at the intersection of Hammond Pond Pkwy. It was reported that a school bus was involved with no passengers on board. Upon arrival, both vehicles were pulled over on the westbound lane of Beacon St. Both vehicles were turning left onto Beacon St. from Hammond Pond Pkwy, which has two left turn only lanes. MV1 was in the left lane, and MV2 was in the right. MV1 attempted to merge with MV2 on Beacon St. but misjudged the turn and drove into MV2's driver's side. MV2 was a school bus contracted by the City of Newton with no passengers on board. Photographs were taken and submitted to IT to be attached to this report. No reported injuries.

On 05/04/19 at approximately 09:47 a citizen called in a vehicle that was parked on Elinor Rd near Woodcliff Rd with heavy damage. The anonymous caller stated the car had been parked there for at least 4 hours. Upon arrival Mass Reg #4FJ861 a 2004 Grey Honda Accord was spotted parked crooked in the road with its back bumper missing, bent drivers side rim and flat tire and large dent in the middle of the front bumper. Dispatch was able to get in touch with the vehicle owner Jaleh Jalili who stated her son Pouya Jalili was driving her car last night. While canvassing the neighborhood of Elinor Rd , Sgt. O'Connell was able to find a tire marks on a lawn and a downed "Sharp Bend" sign in front of #273 Dedham St. This crash site is approximately 1/4 mile away from where the parked car was found. I went to #273 Dedham St and witnessed fresh tire marks that went up on the curb onto the lawn of #273 Dedham St, over a vacant sign post hole and then a "Sharp Bend" sign that was half bent and then resting in the South brake down lane of Dedham St.. There was also a plastic black part of a car along with two (2) metal parts to the under side heat shield of an exhaust. The owner of #273 Dedham St was not home so I left a message on his door regarding this incident. Tody's was called and David showed up and towed the car from the scene. Officer Hagopian went to the residence of Pouya Jalili and spoke with him. Pouya told Officer Hagopian that he was involved in a spin out somewhere on Rt 9. in Wellesley and his bumper became detached. He then pulled into the CVS in Newton at #978 Boylston St where he pulled his bumper the rest of the way off. He then told Officer Hagopian that he continued down Rt 9 until he realized his car wouldn't make it any further and that's when he pulled off and parked on Elinor Rd. He then called a friend to pick him up and drive him home. On the way back to the station I stopped by #978 Boylston St with Officer Benes. He found a Grey rear bumper which is believed to belong to Pouya Jalili's car. I called David Donahue (Tow Driver) and asked that he look under the Honda to see if it had its heat shield. Donahue responded back that Mass Reg# 4FJ861 was missing its heat shield. I then drove to #93 Waltham St and spoke with Pouya Jalili with his mother Jaleh present. Pouya restated the same story he told Officer Hagopian and that he never struck anything in Newton nor was he drinking. Pouya said he might have been in a rush because he wanted to get home before his junior operator curfew. After finishing my interview with Pouya, I went to my cruiser and wrote Mass Criminal Citation #T1444231 for Ch 90/ Sec 24-Leaving the Scene of Property Damage (lawn and sign damage), Ch 90 / Sec 24-Operating to Endanger(putting the public at risk by driving up on the sidewalk) and Ch 89/ Sec 4a- Marked Lanes Violation (crossing Dedham St marked double yellow lines) and handed it to Pouya. I took pictures of the scene where the car was found, the crash site at #273 Dedham St and the bumper found at CVS and placed the disk in an envelope. The envelope was t.o.t. our I.T. Bureau for attachment to this report. Jaleh said her son was returning to Alabama tomorrow to finish high school and then he would be attending The University of Alabama in the Fall. I advised Jaleh about the Criminal Application process. As of today 05/04/19, Pouya has an active Massachusetts License and no B.O.P..Wellesley P.D. was notified of this incident in case they came across new damage on Rt 9.

On 05/20/2018 at 17:30 hours I was notified by Captain Anastasia of a motor vehicle crash involving car, and an electric scooter. Captain Anastaisa stated that the operator of the scooter was a 12 year old male who had sustained unknown injures and had been transported to Boston Children's Hospital via ambulance. Upon my arrival at the crash scene I made the following observations, Putnam and Winthrop streets are public ways in the city of Newton that are two way single lane non divided roadways. The roadway surface was wet from rain earlier in the day. Putnam St runs north to south and Winthrop St runs east to west. There is a stop sign at the end of Winthrop St at Putnam St. I observed MA reg 54MN41 a 2005 Buick LeSabre stopped in the south bound lane of Putnam St approximately 1/2 of the way into the intersection with Winthrop St. I observed approximately three feet of skid marks from the Buick's rear tires. I also observed at the crash scene a Segway Ninebot electric scooter that was on the ground near the Buick(cid:31)s rear passenger side door with its handlebars detached. I spoke with the operator of the Buick, Robert DeSouza who stated that on 05/20/2019 at approximately 16:50 hours he was driving south bound on Putnam St at approximately 25 miles per hour. DeSouza stated that he saw a man, later identified as Jose Estabil standing on the east sidewalk of Putnam St facing Winthrop St who was yelling and waving his arms at something on Winthrop St. DeSouza stated he looked to his right towards Winthrop St and his view was obstructed by a row of hedges. DeSouza stated that as he was looking to his right he began to apply his vehicle's brakes when he saw someone coming at me ". DeSouza stated that he heard a crashing noise from the rear passenger's side of his vehicle. DeSouza stated that he stopped his vehicle after the crash and saw a boy lying on the ground at the rear passenger side of his vehicle. DeSouza stated that he was not injured in the crash. DeSouza's vehicle had minor damage to its rear passenger side door, and did not require a tow. Witness Charlotte Hart stated that at approximately 16:30 hours on 05/19/2019 she was diving south bound on Winthrop St. Hart stated that she had to stop short on Putnam St to avoid a crash " with a boy on a scooter that was racing around on Winthrop St. " Hart sated that she told the boy on the scooter that he should not be ridding the scooter in the street. Witness Jose Estabil stated that he was walking on the east side walk of Putnam St across from Winthrop St. Estabil stated that he saw a boy an electric scooter on Winthrop St approaching Putnam St. Estabil stated that he saw DeSouza's vehicle driving south bound on Putnam St approaching Winthrop St. Estabil stated that the boy on the scooter made no effort to stop, slow or avoid the crash with DeSouza's vehicle. Witness Laurel Fransworth stated that she was driving north on Putnam St approaching Winthrop St when she saw the crash occur. Farnsworth stated a boy on an electric scooter on Winthrop St crash into the rear passenger side door of DeSouza's vehicle. Fransworth stated that she believed that the row of hedges at the end of Winthrop St obstructed DeSouz's vehicle. Rowan Delaney ( 02/27/2007 ) stated that on 05/20/2019 at approximately 16:00 hours he and his fellow F.A. Day Middle School student Thiaugo Rubino ( 03/04/2007 ) were having a race around the area of Winthrop St, Putnam St and Shawn St. Delaney stated that he was on foot running on sidewalks while Rubino was on an electric Ninebot Segway scooter. Delaney stated that Rubion was not wearing a helmet or any other safety equipment. I spoke with Delaney(cid:31)s father Royston Delaney, who stated that the scooter was his and that he was unaware that Rubino was using his scooter, and that he has never seen him ridding it before. Royston Delaney stated that the scooter is battery powered and has a maximum speed of 19 MPH. Royston Delaney stated that his son told him that only Rubino was ridding the scooter. Rubino was transported via ambulance to Boston Children(cid:31)s Hospital. On 05/20/2019 Rubino underwent surgery for a brain bleed. I spoke with Rubion(cid:31)s mother Sonia Rubino who stated that as of 05/21/2019 her son was under observation and in addition to the brain surgery her son had a broken arm, broken leg and a broken back. Based upon witnessed statements and my observations of the crash area I believe that Rubion was at fault for the cause of this crash. Myself and Officer Helms took photos of the crash area and the disks were turned over to the NPD(cid:31)s IT bureau for downloading.

Tipruthai Thangthong stated that on 05/21/2019 at approximately 16:36 hours she was pulling into the driveway of her sisters home at 53 Rangeley Rd while operating her vehicle MA reg 2LVL61 a 2018 Honda Accord. Thangthong stated that her three old son Mighty Mettanant was a passenger in her vehicle. Thangthong stated that she was driving approximately 5 MPH as she drove into the driveway. The driveway runs east to west and Thangthong's vehicle was heading west into the driveway as the sun was in the Western sky creating a glare. Thangthong stated that she did no see anything in the driveway prior to her pulling into it. Thangthong stated that she felt the front of her vehicle strike what she described as " something " and stopped her vehicle. Thangthong stated that she put her vehicle in reverse and then into park and exited her vehicle to see what she had struck. Thangthong stated that she saw her two nieces, Kalla Mongkhonlertsirkul age three and Kelley Mongkhonlertsirkul age seven laying in the driveway in front of her vehicle. Thangthong stated that she then realized that her vehicle had stuck her two nieces as they were in the driveway. Both of the Mongkhonlertsirkul sisters were conscious and alert and were able to walk to an ambulance for transport to Newton Wellesley Hospital for treatment. Thangthong stated that she and her son were not injured in the crash. I observed no damage to Thangthong's vehicle. I responded to the Newton Wellesley Hospital and spoke with the Mongkhonlertsirkul sisters and their mother Piyada Kellum. Kellum stated that her oldest daughter Kelly is a student at the Franklin Elementary School . Kellum stated that her daughters told her that they were playing in their driveway when their aunt's vehicle entered the driveway and did not stop. I spoke with both of the Mongkhonlertsirkul sisters who told me that they tried to get out of the way of their aunt's vehicle but could not in time. The Mongkhonlertsirkul sisters stated that they were both struck in the back and fell to the ground between the two front tires of their aunt's vehicle. Kalla Mongkhonlertsirkul had scrapes to her right knuckles. Kelly Mongkhonlertsirkul had lacerations to her lower left arm and the left side of her face near her eye. I spoke with hospital staff who stated that there did not appear to be any broken bones or other internal injures. I took photos of the crash area and forwarded the disk to the NPD's IT bureau for downloading.

On Wednesday, May 29, 2019 while assigned to Traffic unit N525, I responded to the area of Centre Street and Washington Street, Newton for a report of a motor vehicle crash involving a school bus. The weather at the time of the crash was overcast and the road surface was dry. Centre Street and Washington Street are both public ways maintained by the City of Newton. The operator of MV1, Norma Cedeno (S60008138), stated she was operating a 2016 Dodge Caravan (MA SPN: 104893) around the Washington Street/Centre Street rotary. Cedeno stated as she was traveling in the left turn lane on Centre Street, she attempted to travel straight (N) through the intersection at Washington Street towards 255 Washington Street. Cedeno stated the front passenger side of her vehicle crashed into the driver side of MV2. MV2 was attempted to continue left around the rotary and was located on the passenger side of her vehicle. Cedeno reported no injuries. I observed minor damage to the front passenger side tire area of MV1. Cendeno's passenger, Cristina Velez (S04276767), reported not injuries. The operator of MV2, Mary Sanker (S62890912), stated she was operating her 2016 BMW 328XI around the Washington Street/Centre Street rotary. Sanker stated as she was continuing through the rotary in her lane, the vehicle traveling in the lane to her left (MV2) entered her travel lane in an attempt to travel straight towards 255 Washington Street and crashed into the driver side of her vehicle. I observed moderate damage to the front and rear driver side doors of MV1. Sanker reported no injuries. The operator of MV1 was cited for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care)

VEHICLE #2 HAD EXTENSIVE FRONT END DAMAGE AND MULTIPLE AIR BAG DEPLOYMENT AND WAS ALSO TOWED BY TODYS. OPERATOR OF VEHICLE #1 WAS TRANSPORTED TO NEWTON WELLESLEY HOSPITAL FOR POSSIBLE WRIST INJURY. PASSENGER WAS ALSO TRANSPORTED TO CHECK FOR POSSIBLE INJURIES. OPERATOR AND PASSENGER OF VEHICLE #2 WERE EXAMINED BY MEDICS AND BOTH REFUSED MEDICAL TREATMENT. BOTH OCCUPANTS OF VEHICLE #2 HAD CUTS ON LEGS AND WRISTS ASSOCIATED WITH AIRBAG DEPLOYMENT. THEY WERE ESCORTED TO SCHOOL BY NEWTON NORTH STAFF TO BE EVALUATED BY THE SCHOOL NURSE. On Thursday, June 6, 2019 while assigned to Traffic unit N525, I responded to the intersection of Waverly Avenue and Franklin Street for a report of an accident involving a City of Newton school bus. The weather at the time of the accident was clear and sunny. The road surface was dry. Waverly Avenue and Franklin Street are both public ways maintained by the City of Newton. The operator of MV1, Dorothy Wensink (S40227061), stated she was operating her 2010 Toyota Prius (MA: 538EV6) Eastbound on Franklin Street towards Waverly Avenue. Wensink stated she came to a stop at the stop sign on Franklin Street (E) at Waverly Avenue. Wensink stated as she began to travel through the intersection, a school bus traveling on Waverly Avenue (S) crashed into the rear driver side door area of her vehicle. Wensink reported no injuries on scene. I observed moderate damage to the rear driver side area of MV1. Wensink was able to driver her vehicle away from the scene. MV2 was a City of Newton contracted yellow school bus (MA BUN: 39173) transporting students attending the Ward Elementary School. The bus was returning to the Ward School from a field trip. The operator of MV2, Antonio Goncalves (S54275856), stated he was operating the school bus Southbound on Waverly Avenue towards Franklin Street. Goncalves stated MV1 abruptly accelerated across the intersection from Franklin Street. Goncalves stated he was unable to avoid making contact with MV1 and crashed into the rear driver side of the vehicle. I observed minor damage to the front middle and passenger side bumper area of MV2. The following Ward Elementary School student, teachers, and parents were traveling on the school bus at the time of the accident and reported no injuries. Students: Avishai Arbili, Eleanor Chen, Maeve Donovan, Esat Efendigil, Mark Elentuck, Julian Ismay, Rhea Khanna, Griffin Kirby, Ori Layani, Priscilla Le-Ngo, Callum McLaughlin, Carolina Muniz, Emily Ney, Hrithik Ponduru, Levi Rosenberg Van Gameren, Zachary Schwarzberg, Ava Smoller, Matthew Somers, Lillian Welford, William Balerna, Alexandra Bogdanova, Randi Green, Christopher Iatrou Johnson, Charlotte Nichols, Henry Pozen, Sofia Shadah Alcalde, and Zoey Steinberg. Teachers: Katherine Goldman and Lauren Ludman. Parents: Enrique Shadah, Brian Rosenberg and Xinxin Wang. The Principal of the Ward Elementary School, Becca Brogadir, responded to the crash scene and signed a patient refusal for all of the students with Newton Medics. The students, teachers, and parents, then walked back to the Ward Elementary School with Principal Brogadir. The school bus was able to drive away from the scene. Once the students arrived at the Ward Elementary School, the nursing staff there evaluated each child before they could return to class. The school staff stated they would make notifications to the parents of the children involved in the accident. After speaking with all parties involved, the operator of MV1 was cited in hand for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Pictures were taken of both vehicles and submitted to the IT Bureau.

On 06/30/2019 at 15:30 hours I was notified by Sgt Lee that a crash involving a motor vehicle and a bicyclist had occurred on Wiswall Rd at M Roadway. Upon my arrival at the crash scene I made the following observations, Wiswall Rd is a two lane non divided public way in the city of Newton that runs north to south from a dead end to Dedham St. The posted speed limit on Wiswall Rd is 25 MPH. M Roadway is a two lane non divided public way that runs west to east from Wiswall Rd to a dead end cul-de-sac. The weather conditions at the at of the crash were cloudy, and the roadway surface was wet from rain. I observed MA commercial registration M82794 a gray 2014 Honda Accord stopped in the north bound lane of Wiswall Rd at M Roadway. The Honda is registered to Newton Driving School. I observed two smudge marks approximately four inches in length on the center portion of the front and rear passenger side doors of the Honda. I also observed a 20 inch Kent bike that had be moved from it's final point of rest prior to my arrival. The bike had damage to it's front handle bars, front tire and it's two front forks. The bike's front tire had gray scuff marks on it. At the crash scene I spoke with the operator of the Honda, David Dombrovsky. Dombrovsky stated that he was driving north bound on Wiswall Rd near M Roadway approximately 20 to 25 MPH. Dombrovsky stated that he has had his learner's permit for six months and is a student at the Newton Driving School. Dombrovsky stated that he did not see the bicyclist but heard the impact of the bicyclist stricking the passenger side of the Honda. At the crash scene I spoke with driving school instructor, Igor Drizik. Drizik stated that the Honda was driving north on Wiswall Rd near M Roadway at approximately 20 to 25 MPH . Drizik stated that he did not see the bicyclist until approximately 1 to 2 seconds before impact on the driver's side of the Honda. Drizik stated the Honda is equipped with dual brakes and that he applied the brakes at the same time as Dombrovsky. Drizik stated that he and Dombrovsky got out of the Honda to render aid to the bicyclist. There were three other driving school students in the rear of the Honda at the time of the crash, neither of them stated that they witnessed the crash. None of the occupants of the Honda stated that they were injured and the vehicle did not require a tow. The bicyclist was identified as David Kerwin-Derrick. Kerwin-Derrick was transported via EMS to Boston Children's Hospital form treatment. At the hospital I spoke with one of Kerwin-Derrick's mother's, Patricia Kerwin-Derrick, who stated " We all know that David rode his bike into the car " and that her son had a broken left leg. Kerwin-Derrick is a student at the Memorial Spaulding School. Based upon my observations of the crash scene and statements made to me , I believe that Kerwin-Derrick's unintentionally rode his bike into the passenger side of the Honda. Photos were taken of the Honda, bike and crash scene and the disk was down loaded by the NPD's IT Bureau.

At approximately 1825HRs on August 06, 2019, I was dispatched to 785 Centre St. (Newton Country Day School) in reference to a motor vehicle vs bicycle crash. Upon arrival, I located both parties on the sidewalk adjacent to the intersection of Centre St. and Colby Rd. The operator of the bicycle was complaining of feeling dizzy, although she stated she did not hit her head, and suffered several abrasions on her left leg and hands; the operator of MV1 stated she was uninjured. The operator of MV1 stated that as she was traveling northbound on Centre St., she attempted to give the operator of the bicycle room, but the bicyclist made an abrupt left hand turn and struck MV1. The operator of the bicycle stated she was traveling northbound on Centre St. in the Bicycle Lane. She then signaled a left hand turn and entered the Motor Vehicle Travel Lane to make a second left hand turn to travel eastbound on Colby Rd. Once she exited the northbound Motor Vehicle Travel Lane on Centre St. and began to cross over the southbound lane, MV 1 crossed over the solid yellow lines onto the southbound side of Centre St. and struck her bicycle. MV1 sustained damage to its right front bumper and I noted red paint transfer of the left side of the front bumper, which the operator of MV1 states was old damage. MV1 also sustained a flat left front tire after contacting the curb. The bicycle sustained damage to both tires and where the rear tire attaches to the frame appeared to be warped. I took several photos of the damage and have submitted them to be attached to this report. The operator of the bicycle was transported to the hospital by Newton Medics and the operator of MV1 signed a patient refusal. Todys Services arrived and took custody of MV1, for which I completed and filled an NPD Towed Motor Vehicle Form. All streets mentioned are public ways in the City of Newton. Due to the potential danger of serious bodily injury caused by the operator of MV1's reckless driving behavior (crossing the solid yellow road divider line and driving on the wrong side of the road against traffic), the actual damage caused (striking and causing the bicycle operator to be transported to the hospital and damage to the bicycle) and that the bicycle operator was clearly visible/in clear contrast with the road (riding a red bicycle and wearing a red shirt) I issued the operator of MV1 MA Criminal citation T1445010, in hand, for violating one count of MGL Ch.90 S. 24 (Operating To Endanger) I cleared without incident. Note: I logged the bicycle into property/evidence for safekeeping, as it could not be taken on the ambulance.

No injuries and all adults signed refusals with the medics. Five of the passengers in MV2 were minors. The Director of Clearway School arrived on scene and signed refusals on behalf of all the minors. The Director, Peter Rosen, called to notify all parents before his arrival. He gathered all students and drove them back to Clearway School for parents to pick up from there. MV1 operator, Michael OKeefe, was given MA Uniform Citation T1442827 in hand for 90/14B - Fail to Yield at an Intersection.

On Friday, September 13, 2019 at approximately 16:19 hours, while assigned to N496, I responded to 162 Warren St. for a report that two boys were hit by a car earlier in the day. Once on scene I spoke with Wes Moore and Luca Toro (both attend Oak Hill Middle School), as well as their mothers Jamie Moore and Alyssa Toro. Wes and Luca explained that at about 15:15 hours, they were crossing at the crosswalk adjacent to the Bank of America in Newton Centre (808 Beacon Street), heading towards the Newton Centre municipal parking lot. The boys stated that as they crossed a large van, which was traveling westbound on Beacon Street in the left turn lane, stopped to let them cross. Wes and Luca stated that they did not realize that there was another westbound travel lane, and after passing the truck they were both hit by an unknown sedan. Wes and Luca stated that the driver, who they described as an approximately 50 year old white female, got out of her car and started yelling at them to be careful. Wes and Luca stated that the unknown female operator checked to see if they were hurt and then left the area heading westbound on Beacon Street. Wes and Luca stated that they both fell to the ground as a result of being hit by the unknown motor vehicle, but only sustained minor injuries. I offered to have an ambulance respond to evaluate Wes and Luca, however, Mrs. Moore and Mrs. Toro declined. I advised the boys and their mothers that I would be documenting this incident with a crash report, and that I would head to the area to see if Bank of America had and cameras facing the crosswalk, so that we could hopefully identify the operator of the unknown motor vehicle. I then responded to the Bank of America, however, the bank was closed for the day. Supplement by Officer Ferguson: On Monday 09/16/2019 at approximately 0900 hours I responded to the Bank of America and spoke with the employees who stated they do not have any cameras that record the street or sidewalk areas. The camera they have only records the front door and lobby area.

The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave making a left turn onto Morton St when he was struck by MV#2. The operator of MV#1 stated he was unsure of where MV#2 came from. There was a strong solar glare facing westbound on Commonwealth Ave during this accident. MV#1 sustained heavy driver's side front end damages. Both front airbags were deployed. The operator of MV#1 stated he was not injured. The front passenger of MV#1 was not injured and had already left the scene to attend school. The driver's side rear passenger sustained a bleeding nose from this accident. She was unsure what caused her injuries because it happened too quickly. She was evluated by Newton Paramedics and signed a patient refusal of treatment. The initial point of impact for both vehicles was in the middle intersection of Commonwealth Ave at Morton St. There were broken vehicle parts and tire skid marks starting from the point of impact and onto the berm on Commonwealth Ave on the carriage lane side. MV#2's final resting spot was directly accross from 808 Commonwealth Ave. MV#2 sustained heavy front end damages to its driver's side wheel (Broken axle) and flat front right side tire. MV#2's driver's side airbag was deployed. The operator of MV#2 was transported by Newton Paramedics to the Boston Medical Center. I spoke to the operator of MV#2 via telephone and she stated she was travelling westbound on Commonwealth Ave when she was struck by MV#1 and caused her swerve onto the carriage lane berm. The operator of MV#1 is 6.5 months into her pregnancy and sustained neck, back, bruises and left hand injuries. She was transported to Boston Medical Center and was later discharged the same day. She was provided with this accident report number and was advised to contact her insurance provider. I spoke to the witness who was at the intersection of Cedar St and Commonwealth Ave. The witness stated MV#1 was travelling eastbound on Commonwealth Ave and had stopped, attempting to make a left turn onto Morton St. The witness stated she then observed MV#2 travelling westbound on Commonwealth Ave and struck MV#1. The witness stated she believed MV#2 was travelling at a high rate of speed because it continued to travel onto the berm after striking MV#1. I took digital photographs of the damaged berm on Commonwealth Ave. Based on the statements made to me, I issued in hand to the operator of MV#1 citation #T1268357 for violation of c89 s8 failure to yield when turning left. Both vehicles were towed by "Tody's Towing".

On Tuesday November 12th, 2019 at approximately 1224 hours while working in N526 I responded to the intersection of Derby Street and Waltham Street for a motor vehicle vs. pedestrian accident. At the time of the accident the weather conditions were cloudy and rainy, and the road surface was wet. Both Derby Street and Waltham Street are public ways owned and maintained by the City of Newton. Upon arrival I spoke with the pedestrian identified as Helen Makadia (S35538182) who stated she was walking in the crosswalk on Waltham Street heading towards Derby Street when she was struck by MV1. Helen stated she was able to walk to the side of the road after impact but complained of knee pain. Medic 2 responded and Helen signed a patient refusal. Sgt. Lee transported Helen to the Franklin Elementary School where she was picking up her children and then she left on her own accord. The operator of MV1 identified as James Bond (S93760526) stated he was taking a left turn from Derby Street onto Waltham Street and did not see the pedestrian in the crosswalk until he struck her with his vehicle. James was very remorseful and acknowledged he was at fault. James reported no injuries and had no damage to his vehicle. I issued James in hand a Mass Uniform Citation #T2012435 for Chapter 89 Section 11 for failing to stop for a pedestrian in the crosswalk.

The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. The operator of MV1, Jessica Moore, stated she was backing out of a parking spot in the rear of the Angier Elementary School while operating a 2007 Chrysler Town and Country (MA:395KR9). Moore stated she did not see MV2 passing by as she was backing out and the rear driver side bumper area of her vehicle crashed into the rear driver side of MV2. Moore reported no injuries. I observed moderate damage to the rear driver side bumper area of MV1. The operator of MV2, Patrice Lys, stated he was traveling Westbound in the parking lot of the Angier Elementary school in a 2018 Ford EcoVan (MA MVN: M2546A). Lys stated a vehicle on his driver side backed out of a parking spot and the rear end of MV1 crashed into the rear driver side of MV2. Lys reported no injuries. I observed minor damage to the rear driver side area of MV2. The operator of MV1 was cited with Massachusetts Uniform Citation T2016428 for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care, Backing). Photos were submitted to the IT Bureau.

The crash forced her car to spin sliding over the center island coming to final rest in the EB lane of Washington St. I was approached by Newton School Crossing Guard Regina GAYLE who stated she had witnessed the crash. GAYLE stated the Operator of the black Jeep Ma Reg 8VX486 had the green light, was well into his left turn from the EB laft lane of Washington St onto Beacon St when the Suburu "came flying " through the intersection. GAYLE stated she didnt know if the Suburu driver was trying to beat the light but she was "going so fast". I spoke with the operator of Ma Reg 8VX586 , nicholas MANCINI. MANCINI stated he was taking the left onto Beacon from EB Washington St with the green light when the other operator came over the rise of WB Washington St at a extremely high rate of speed . On 11/26/19 at 14:11 hours, I responded to Newton South High School located at 140 Brandeis Road for a two car motor vehicle accident involving a school transport van for the City of Newton and another vehicle. No injuries were reported. The road conditions at the time were dry and the weather was a mix of sun and clouds. Traffic conditions were heavy due to the release of school at the time. Operator of MV# 1 states he was operating straight ahead eastbound on Brandeis Road awaiting the backed up traffic in front of him that was entering the right roundabout for the main entrance to the high school. While doing so, he states that MV # 2 came up and passed him on the left to go around the traffic. MV # 2 then clipped his vehicle on the driver's side front quarter panel while passing him. Operator of MV # 2 states she was also operating straight ahead eastbound on Brandeis Road awaiting the backed up traffic in front. She states that once there was an opening in the roadway ahead of her, she signaled her left directional and started to pass traffic on the left. While doing so, she says that MV # 1 suddenly pulled out to the left without signaling and struck her vehicle on the front passenger side. I observed minor scuff marks to the front passenger quarter panel area and slight damage to the metal grate footstep on the passenger side. The operator was also transporting two students for the City of Newton through JSC Transportation Services at the time. Both students attend Newton South High School. On Thursday, January 23, 2020 while assigned to Traffic unit N525, I responded to the intersection of Brookline Street and Hartman Road, Newton for a report of a motor vehicle/pedestrian crash involving a City of Newton Crossing Guard. The weather at the time of the crash was overcast. The road surface where the crash took place was dry. Brookline Street and Hartman Road are both public ways maintained by the City of Newton. On arrival, the injured pedestrian was in the process of being transported to Newton Wellesley Hospital with minor injuries. I spoke with the operator of the vehicle involved in the crash, Menachem Uminer (S43996134). Uminer stated he was operating his vehicle on Brookline Street (S) towards Hartman Road. Uminer stated he was stopped in traffic on Brookline Street prior to the crash. Uminer stated the vehicle in front of him proceeded to travel straight from it's stopped position and he then began to follow the traffic on Brookline Street. Uminer stated he did not see anyone in the vicinity of his vehicle as he began to travel straight from his stopped position. Uminer stated as he was traveling by Hartman Road, the front passenger side area of his vehicle made contact with a pedestrian on the right side of Brookline Street at Hartman Road. Uminer stated he did not see this party until after they were struck and he then proceeded to pull his vehicle over. Uminer stated he was operating his vehicle at a very low speed at the time the collision took place. After speaking with Uminer, I observed the white 2017 GMC Yukon (MA: 9NX525) involved in the crash parked on Brookline Street (S) just past Hartman Road. Uminer stated the vehicle had been moved after the collision took place. I observed no obvious damage to the body of the vehicle on the front passenger side area where the collision took place. The front passenger door mirror was folded inward towards the door. Uminer stated he believes the front passenger door mirror was the point of contact between his vehicle and the pedestrian. I spoke with a witness to the crash, Ashley Garcia (S94193596). Garcia stated she was attempting to cross Brookline Street at Spaulding Lane. Garcia stated she entered the marked crosswalk and was a few steps into Brookline Street when a black colored vehicle traveling on Brookline Street (N) slammed on its brakes near the crosswalk. Garcia stated she looked to her left at the vehicle and then heard a thud to the right from the area where the crossing guard was standing near Hartman Road. Garcia stated she looked over and saw a crossing guard on the roadway on Brookline Street (S) at Hartman Road. The injured pedestrian involved in the crash is Cassandra Alexander (S23636307). Alexander is a crossing guard for the City of Newton and is assigned to the intersection of Brookline Street and Hartman Road for the Memorial Spaulding school crossings. This crash occurred while Alexander was performing her official duties. Alexander was wearing her department issued yellow reflective jacket at the time of the crash. I spoke with Alexander at Newton Wellesley Hospital where she was being treated for her injuries. Alexander stated she was performing her duties as a crossing guard at the intersection of Brookline Street and Hartman Road. Alexander stated she had just finished crossing two pedestrians in the marked crosswalk on Hartman Road when observed a pedestrian enter the crosswalk on Brookline Street at Spaulding Lane. Alexander stated she began to enter the intersection of Brookline Street at Hartman Road with her hands up and her department issued yellow reflective jacket on in an effort to stop traffic in the area to allow the pedestrian in the marked crosswalk safely cross. Alexander stated she was struck on her left side by a passing vehicle. Alexander stated she did not see the vehicle and fell to the roadway. Alexander stated as she was falling to the roadway she extended her right arm in an effort to brace her fall. Alexander stated she had injuries to her arms and legs and was sore from the collision. I spoke with a second witness to the crash via telephone, Anand Vaidya (S43996134). Vaidya stated he was stopped in traffic behind MV1 on Brookline Street (Southbound). Vaidya stated he observed the vehicle in front of MV1 proceed straight on Brookline Street after being stopped in traffic. Vaidya stated he then observed MV1 proceed straight on Brookline Street at Hartman Road at a low rate of speed. Vaidya estimated the speed of MV1 at this time to be 5 miles per hour. Vaidya stated a crossing guard was on the right side of the roadway on Brookline Street at Hartman Road wearing a yellow and black reflective jacket and snow pants. Vaidya stated the crossing guard had her back towards him and MV1, and extended her arm out to stop traffic. Vaidya stated at this time MV1 rolled by the crossing guard at a low rate of speed and the front right fender area of the vehicle hit the left rear leg/lower back area of the crossing guard. Vaidya stated the crossing guard bounced off of the car and fell to the roadway. Vaidya stated he believes MV1 was unaware he hit the crossing guard and observed the vehicle travel another 10 feet until it came to stop on Brookline Street. Vaidya stated the crossing guard was not in a marked crosswalk at the time of the crash. After speaking with all parties involved, Uminer will be cited with Massachusetts Uniform Citation T0115905 for Chapter 90, Section 14 (Failure to slow for a pedestrian in the roadway). Photos were taken of the vehicle and scene and submitted to the IT Bureau.

On 02/02/20 at approximately 14:10 I met with Bobson Berleus who parked his fathers car (Mass Reg#542DV3 -2003 Toyota Camry Grey) in a parking spot outside of Boston Ballet School at around 06:00 while he went to work inside T.J. Maxx. When Berleus returned to the vehicle at around 14:00, he found that it had been struck on the front drivers side fender. I spoke with Stop & Shop Manager Jim Boucher (where Berleus also works) and he was going to look at security footage of today and try to see if he could locate the accident. There are no cameras from the Ballet School in the direction where Berleus parked. ************************UPDATE*************************** As of my 02/09/20 at approximately 11:00, I have yet to hear back from Manager Jim Boucher or anyone else from Stop & Shop. I will now consider this investigation closed unless I am contacted in the future with new information.

On Friday, March 6, 2020 while assigned to Traffic unit N525, I responded to the area of the Countryside School (191 Dedham Street, Newton) for a report of a motor vehicle crash involving a City of Newton contracted school bus. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with the operator of MV1, Gianna Gemelli (S95586165). Gemelli stated she was traveling Southbound on Parker Street towards the Boylston Street (E) on ramp in a JSC Transportation owned 2016 Dodge Grand Caravan (MA SPN: 105641). JSC Transportation is contracted by the City of Newton to transport students through out the city. Gemelli stated as she was traveling on Parker Street, MV2 attempted to pass her on her driver side to take a left onto Boylston Street (E). Gemelli stated the front passenger side of MV2 made contact with the rear driver side of MV1. I observed minor damage to the rear driver side bumper area of MV1. Gemelli was transporting two students to the Countryside Elementary School at the time of the crash. The students in the vehicle were identified as Mayson Martinez and Hailey Nesvacil. No injuries were reported by any of the occupants of MV1. Countryside Principal Elizabeth Herlihy signed a patient refused for Mayson Martinez. Martinez's mother Rosa Mordan was notified of the crash by school staff. Gemelli signed a patient refusal for her daughter, Hailey Nesvacil. Regina Moody was notified of the crash. I spoke with the operator of MV2, Clary Binns (s100112165). Binns stated she was traveling Southbound on Parker Street towards the Boylston Street (E) on ramp. Binns stated she attempted to go around MV1 to enter the on ramp to proceed onto Boylston Street when the front passenger side of her vehicle made contact with the rear driver side of MV1. I observed minor damage to the front passenger side fender/wheel area of MV2. Binns reported no injuries. Prior to my arrival, Binns stated she had two passengers in her vehicle at the time of the crash that had left the scene. They were identified as Maya Bock and Jonatan Freedman-Naditch. I contacted parents for both parties to notify them of the crash. No injuries were reported. Binns, Bock, and Freedman-Naditch are all students the Meridian Academy in Boston. Clary Binns was provided with Massachusetts Uniform Citation T0115909 for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Photos where taken of both vehicles and submitted to the IT Bureau. Parker Street and Boyston Street are both public ways in the City of Newton.

On 7/3/20, I met a party in the lobby of the NPD for a past hit and run report. The RP, Aleksander Duri ( MA Reg: 2LG632) stated he was parked in the Wegmans parking garage on 6/27/20. When Duri returned to his vehicle there was a note on it that said "I just saw a car hit your driver side back bumper. Teenage boy was driving, license plate 159XT5 Blue Subaru outback. If you have damage and need a witness you can call me [phone number in witness section]." MA Reg: 159XT5 comes back to a 2013 blue Subaru Outback registered to Emily Franklin-Strauss. In the census it lists three males who reside at the address in the same age group (DOB of 11/8/2001, 5/20/2004 and 7/2/1999). I attempted to call the phone number on file with negative results. There are cameras in the Wegmans garage however it did not capture the accident. Traffic Bureau update (Officer Gaudet): On Monday, July 6, 2020, I attempted to make contact with the registered owner of MV2, Emily Franklin-Strauss (S56184971), with a negative result. A Hit and Run inquiry was mailed to Ms. Franklin-Strauss. On Wednesday, July 15, 2020, I spoke with the operator of MV2, Samuel Strauss (S90135443), via telephone. Strauss stated he was operating MA: 159XT5 at the time of the crash in the parking garage. Strauss stated he was backing his vehicle up and heard a sound. Strauss stated he got out of his vehicle and looked for damage on the parked car he backed up into. Strauss stated he stayed on scene for 7-10 minutes and left after he observed no damage. Strauss stated a claim has been filed and insurance is now involved. Strauss stated he is an inexperienced driver and did not know what to do in a situation like this. Strauss was advised that if he is invovled in a crash in the future, he should contact the local police department in the city the the crash occurs in so the police can help faciliate an exchange of information. Strauss stated there was another passenger in the vehicle with him at the time of the crash named David Banteekhan. Strauss could not provde any other information for this party except that he lives in Newton and his Snapchat name is DDavid5935. I could not find any other information on this party. No injuries reported. Strauss is a student at Newton South High School. No futher action will be taken.

On Wednesday September 2nd, 2020 I responded to 341 Lexington Street for a report of a past motor vehicle accident involving a pedestrian that reportedly occurred around 12:25pm at the intersection of Commonwealth Ave and Lexington St. At the time of the reported incident the weather was cloudy/rainy and the road surface was wet. Both Commonwealth Ave and Lexington St are owned and maintained by the City of Newton. Upon arrival I spoke with the parent of the pedestrian identified as Tamika Olszewski. Tamika stated her daughter, Morgan Olszewsk, her twin brother Dean Olszewski and several friends took their bikes and went to Dunkin Donuts in Auburndale. Tamika stated when the group returned from Dunkin Donuts her daughter Morgan told her that she was hit by a car while crossing Commonwealth Ave. I spoke with Morgan and she stated she was in front of Dunkin Donuts and pressed the crosswalk button on the traffic light to cross over Commonwealth Ave. Morgan stated she crossed the street inside the crosswalk and noticed the crosswalk light signal was counting down. Morgan stated she heard a shrieking noise from a car slamming on the brakes and stuck hand out and touched a white car to keep her balance. Morgan stated she did not fall down or sustain any injuries. Morgan stated she continued across the street towards the picnic benches that are located in front of Starbucks. Morgan stated she then walked home and told her mother what happened. Morgan's only description of the vehicle involved was white car. Dean Olszewski and Ryan Poras stated they were at the picnic benches and left their bikes there and walked in the crosswalk to Dunkin Donuts to get some food. Dean and Ryan stated they then left the store and crossed back to the picnic benches where Deans sister Morgan was waiting. Dean gave Morgan some money so she could go to Dunkin Donuts and Morgan went and crossed the street alone and the next thing they saw was Morgan in front of Dunkin Donuts with an unknown woman with her arms around Morgan. Dean and Ryan stated Morgan crossed back to the picnic benches and when they asked Morgan what had happened she stated "nothing". Later Morgan then told Dean she was hit by a car. I spoke with an employee of Dunkin Donuts who stated he witnessed the entire incident. Kelvin Matovu stated he had groups of kids coming inside to get donuts around 12:25pm. Kelvin stated he witnessed Morgan with other children at the picnic benches in front of Starbucks. Kelvin stated he saw Morgan attempting to cross Commonwealth Ave with her helmet on and no bicycle with her. Kelvin stated Morgan pressed the crosswalk button on the traffic light at the intersection but stated she did not cross in the crosswalk and crossed directly in front of the stores windows which are not in line with the crosswalk. Kelvin stated he did not notice if she had the walk sign or not. Kelving stated as Morgan crossed into traffic she was rushing and stopped in the middle of the road where the two yellow medium stripes are and looked confused. Kelvin stated it seemed she was confused because cars were going past her in both directions and so she stopped in the middle of the road. Kelvin stated a female customer who was inside the store went out and made contact with Morgan when she finally crossed the street and made it to the front door of Dunkin Donuts. Kelvin stated he did not see a car make contact with Morgan or hear anything related to a crash. Kelving stated Morgan seemed confused and did not appear injured. Kelvin stated the female customer put her arms around Morgan to make sure she was okay. After the incident, Kelvin stated Morgan did not enter the store and went back to the picnic benches where the other children were located and they left northbound on Lexington Street. Kelvin stated the female customer came into the store and Kelvin gave the woman a free coffee for assisting Morgan. The unidentified female customer then left the area. Dunkin Donuts has no cameras that face the street. Officer G. Claflin spoke with the employees at Keys Drugstore and they stated they did have cameras facing the street and will contact the store manager who will contact the Traffic Bureau if the incident was caught on the camera. I advised Tamika that a report will be filed and if there is any development regarding the possible car involved I will contact her. Medic 2 was on scene and Tamika signed a patient refusal for Morgan. Morgan is a student at Burr Elementary School. While writing this report Tamika called and stated that Morgan remembered that the woman who assisted her after the collision walked her inside Dunkin Donuts.

On November 1st, 2020 at approximately 14:02 hours while assigned to N491 I responded along with Newton Fire and Medic One to the intersection of Pearl St @ Jackson Rd for a MV crash with minor injuries. On my arrival I first observed in the intersection a green colored Volkswagon with Vermont tag,HPK270 with extensive damage. The other involved vehicle a white Jeep Wrangler, bearing Ma tag, 8WK846 was located on the property of the Lincoln Elliot School, 191 Pearl St, also with extensive damage. The operator of the green Volkswagon was identified as Viorel Marin. He stated he was going E/B on Pearl St and was approaching the intersection of Jackson Rd when the light had turned green. As he was entered the intersection vehicle #2 came from his right side(N/B) on Jackson Rd at, according to him, a high rate of speed causing his vehicle to collide with the Jeep. He doesn't remember much after that. I observed a small cut to his right knee and later he showed me a small abrasion to his forehead. All his air bags had deployed on contact with vehicle #2 . I located the operator of the Jeep who was sitting on the ground being assisted by an unidentified women(not a passenger). She appeared shaken but had no visible signs of any serious injury from the crash. She identified herself as Tara Feeley. According to Ms Feeley she had been travelling N/B on Jackson Rd and was attempting to continue straight across the intersection of Pearl St when she was struck by vehicle #1 in the intersection. The impact caused her vehicle to flip over unknown amount of times finally landing on the property to the Lincoln Eliot School. Based on operator #2's statements, my observations of the scene, damage to her MV, it appeared her vehicle after contact with vehicle #1, flipped over N/B across the intersection first hitting the traffic light on the northeast corner, additionally making contact with the utility pole just east of the traffic light before finally landing right side up on the property of the Lincoln Eliot School. She reported climbing out her passenger side window when her vehicle finally stopped. She said,"she was lucky she had her seat belt on". I agreed. Ms Feeley also reported that she was sure that her light was green as she entered the intersection. I attempted to locate any possible witnesses from the crowd that were gathered near the crash site. There were two that heard the crash but none that actually saw what happened. I couldn't locate any cameras near the intersection. Both vehicles were towed from the scene due to the extensive damage/both totaled. Both operators had minor injuries and both declined to be transported to the hospital. They signed patient refusals. Officer Danny Anderson later took several pictures of the crash scene and the all the property damage. Both utility companies were notified. The camera disc was placed in the technology bureau mailbox.

Operator of vehicle one Timothy Trencher stated that on 12/01/2020 at 08:36 hours he was driving his vehicle a 2010 Toyota Prius east bound on Linwood Ave near Walnut St. Linwood Ave and Walnut St are public ways in the City of Newton. Techler stated that he came to a stop at the stop sign at Walnut St and then turned left into the north bound lane of Walnut St. Techler stated that he did not see any on coming traffic , bicyclist or pedestrians in the roadway before he turned into the north bound lane of Walnut St. Techler stated that his vehicle's front bumper made contact with a bicycle ridden by Nicolas Le ( DOB 04/05/2006 ) . Techler stated that he was not injured and his vehicle did not require a tow. Le stated that he is a student at Newton North High School and was riding his bike a Giant mountain bike to school. Le stated that he was stopped on his bike on the northeast corner of the intersection of Linwood Ave and Walnut St. Le stated that he saw a pedestrian latter identified as Jason Kaufman walking west bound in the crosswalk from the east side of Walnut St to the west side of Walnut St. Le stated that as Kaufman was crossing over Walnut St he assumed that it was clear to cross over Walnut St while riding his bike. Le stated that he was unsure if the yellow flashing crosswalk lights were activated. Le stated that he rode his bike off of the side walk at the northeast corner of Walnut St at Linwood Ave. Le stated that he did not ride his bike in the marked crosswalk that run from east to west over Walnut St at Linwood Ave. Le stated he rode his bike diagonally into the intersection towards the south west corner of the intersection. Le stated that he did not see Techler's vehicle prior to entering the intersection. It should be noted that at the time of the crash Le was wearing earphones and stated the he was listening to music. Le stated that he was not injured in the crash and signed a patient refusal from in the presence of his father Thanh Le who responded to the crash scene. Le's bike had damage to the front tire and front breaks. Based upon statments made to me by Le, Techler and my observations of the crash area, I believe that they both bear responsibility for the cause of the crash. Le was not in a marked crosswalk and was not sure if the flashing yellow crosswalk lights were activated. Le also was listening to earphones while crossing diagonally into the intersection. I issued Techler MA Unifrom Citation T2015904 and cited him for a violation of MGL 90/14 , failure to yield to a bicyclist while turing left.

HE STATED HE BACKED BUT DOES NOT RECALL HITTING ANY VEHICLE. HE STATES HE BACKED VERY SAFELY AND HAS A REAR VIDEO CAMERA AS WELL AS A DISTANCE ALARM. BOTH VEHICLES HAD NO VISIBLE DAMAGE. On Friday, January 29, 2021, Mr. Jose Bonilla (S73276930) stated he was operating a 2016 Thomas School bus (MA: SB40311) owned by Eastern Bus Company at the intersection of Crafts Street and the driveway to 34 Crafts Street, Newton. Crafts Street is a public way maintained by the City of Newton. Mr. Bonilla stated he proceeded to take a right turn onto Crafts Street from the driveway and the passenger side of his vehicle made contact with a fire hydrant. The crash caused the hydrant to fall to the roadway. No injuries reported. City noticed of damage. Photos of damage were submitted to the IT Bureau.

Operator Diego Colindres states he was stopped at the stop sign located on Central St westbound at Grove St. As he proceeded through the sign to drive onto Grove St, a female jogger ran out into the street from the sidewalk in front of 16 Grove. Colindres reports he did not see the jogger until his front bumper made contact with the jogger's leg, which in turn knocked her to the ground. The jogger then got up, said she was ok, and tried to continue on her way. Colindres asked her to stay on scene and wait for emergency personnel, to which she obliged. It should be noted that there are bushes located in front of 16 Grove St which substantially hinder the view of the sidewalk for vehicles approaching Grove from Central. Jogger Rachel Coffin confirmed that she was crossing Central St heading southbound on Grove St when she was hit by vehicle #1. Coffin was not sure if she stopped prior to crossing the street, and did not see the vehicle until contact was made. She does recall that the vehicle was traveling at a slow rate of speed when she was struck. Coffin's attire consisted of dark colored shorts, a navy t-shirt, and white sneakers. I observed a light red area on Coffin's outer left thigh consistent with the height of vehicle #1's front bumper. She was tended to by the medics and declined transport, repeatedly saying that she just wanted to get back to school (Lasell) and do her homework. Due to Coffin's uncooperativeness and multiple requests to leave the scene, pictures of her minor injury were not taken. There was no visible damage to vehicle #1. Traffic responded, however Coffin wished to leave prior to arrival.

On Wednesday March 31, 2021 at approx 1430 hours Officer Panica responded to the area of Washington Street and Jackson Road for a report of a tractor trailer (car carrier) with New York registration that knocked over a traffic signal and kept traveling east on Jackson Road with out stopping. Officer Nardelli and I responded to the area as dispatch was giving updates of tractor trailer information and direction of travel. I traveled west on Watertown Street from Chapel Street and observed a car carrier traveling west approx four blocks west ahead of me. I activated my emergency lights and made a motor vehicle stop at Watertown Street at Nevada Street. I observed a trailer NY reg # BH32997 which dispatch confirmed this reg was the one involved in the crash. The name of the comapny is DIVERISIFIED AUTOMOTIVE INC of 6 RAMLAND ROAD ORANGEBURG N.Y. 10962 N.Y. apportioned tractor reg 17833PB / trailer NY reg BH32997, the driver was later identified as MA lic # SA0310636 Januel Ortiz d.o.b. 5-17-2001 of 111 Country Club BLVD apt. 201 Worcester MA 01605. Officer Panica confirmed the crash site location and damage to a City of Newton Traffic signal/footings/ sidewalk/retaining wall/landscape. Officer Panica was also approached by witness Erin McGinty of 233 Jackson Road(see statement below) I approached the car carrier on the passenger side and observed fresh damage on the trailer that would be consistent with striking the traffic signal. I then approached the driver of the car carrier Mr. Ortiz. I advised him of the crash and he stated he was traveling west on Washington Street and turned right (north) onto Jackson Road. He stated while turning his trailer struck the traffic signal knocking it over. I asked him why didn't he stop to assess damage or notify anyone. He stated he was looking for a safe place to pull over. He stated he continued north on Jackson Road then left onto Pearl Street and then left onto Watertown Street to drop off vehicles at the 445 Watertown Street,which is where I Pulled him over. I advised him I am familiar with the travel route he went and the length of Jackson Street is not a Heavily traveled area since the school is closed from the crash site, and he traveled approx a mile and a half until I pulled him over. I advised him I would be doing a level III inspection and advising him of my findings. Witness Ms. McGinty stated she heard the crash and observed the car carrier with NY reg # BH32997 have contact with the traffic signal and continue to travel north on Jackson Street without stopping. Therefore after observing damage/pictures and statements I issued in hand MA criminal citation # T2017488 in hand to Mr. Ortiz for Chapter 90 Section 24 LEAVING THE SCENE OF CRASH WITH PROPERTY DAMAGE City Ordinance 19-75 FAILURE TO USE CARE IN TURNING. Mr. Ortiz was explained the process. I took pictures of the damage to the car carrier, and crash site area that I will have downloaded at the I.T. Bureau.

On Wednesday April 7th, 2021 I responded to 309 Crafts Street for a report of a motor vehicle accident involving a City of Newton contracted school bus (mini-van). At the time of the accident the weather was cloudy and the road surface was dry. Crafts Street is operated and maintained by the City of Newton. Upon arrival I spoke with the operator of the school bus identified as Sybil Berry (S93963801). She stated she had just turned left from Walnut Street onto Crafts Street and was traveling eastbound when a parked vehicle on Crafts Street pulled into the travel lane at the same time as she was traveling straight, and made contact with her vehicle. Sybil's vehicle sustained minor passenger side panel damage. Photographs of the damage were taken and submitted to the IT Bureau. Sybil stated the operator of the motor vehicle couldn't wait for police to respond but he left his information. The operator of the other vehicle involved was identified as Jun Nakmoo. I was able to speak with Jun but there was a strong language barrier. Jun stated he was parked on Crafts street and did not see any vehicles coming his way so he pulled into traffic. Jun stated the mini van appeared out of nowhere and sideswiped his car. Jun reported minor damage to his drivers side panel. Both vehicles were able to drive safely from the scene. The school bus had two children passengers in the vehicle at the time of the accident. The children were identified as John Castellano (07/31/09) and Delsi Castellano (06/26/11). Both students were not injured and were picked up by their mother Adria Castellano (01/05/81). The children both attend Bowen Elementary School and were being transported to Waltham.

On 04/29/2021 at 15:22 hours I was dispatched to 53 Brentwood Ave for a report of a motor vehicle crash involving a bicyclist. Upon my arrival, I made the following observations of the crash scene. Brentwood Ave is a two way non divided public way in the City of Newton that runs east to west from Greenlawn Ave to Walnut St. Brentwood Ave is a thickly settled residential area with a 25 MPH speed limit. The roadway conditions at the time of the crash were wet due to rain. I observed MA reg 111ZS1 a 2020 Honda Civic parked on the side of Brentwood Ave just passed the intersection of Judith Rd facing westbound. I observed that the Honda had damage to it's front driver's side bumper. When I inspected the Honda I observed that the driver's side headlight casing was cracked and a piece of it was missing. I further observed an approximately six inch in diameter spider web like crack in the lower driver's side windshield. The windshield crack was inward , indicating that an external object had caused the damage. There was another single crack onn the driver's side windshield that extended from the hood to the roof. The operator of the Honda , Lisabeth Ioannilli stated that on 04/29/2021 at 15:20 hours she was driving westbound on Brentwood Ave near the intersection of Judith Rd. Ioannilli stated that she estimated that the speed of her vehicle was approximately 25 to 30 MPH. Ioannilli stated that she did not see a bicyclist in her path of travel until the point of impact when her vehicle was aproximately 3/4 of the way passed the intersection of Judith Rd. Ioannilli stated that her vehicle's front driver's side bumper made contact with the bicyclist. Ioannilli stated that the bicyclist was tossed off of her bike and onto the hood and windshield of her vehicle before rolling onto the ground. Ioannilli stated that she pulled her vehicle to the side of the road and went to check on the bicyclist. Judith Rd is a public way in the City of Newton that runs east and then turns south from Walnut St to Brentwood Ave. There are no traffic control devices at the intersection of Judith Rd and Brentwood Ave. Ioannilli stated that she was not injured in the crash and her vehicle did not require a tow. At the crash scene, the bicyclist , Lucia Campbell, a student a FA Day Middle School ( DOB 11/08/2006 ) was being treated in an ambulance and was transported to Boston Children's Hospital for treatment. Campbell was conscious and was able to answer questions from EMS members. Campbell stated that she was wearing a helmet at the time of the crash and that she rode her bike south on Judith Rd and was turning eastbound onto Brentwood Ave when the crash occurred. I spoke with Campbell's parents Adam Campbell and Iris Delasota who stated that their daughter was under going x rays and other tests to determine if she has internal injuries. I took photo's of the bike, Honda and crash scene and the disk was downloaded by the NPD's IT Bureau. Based upon my investigation of this crash I concluded that Ioannilli was at fault for it. I issued via US mail Ioannilli MA Uniform Citation T1446633 and cited her for a violation of MGL 90/14 failure to use care while approaching a bicyclist. Campbell's bike was turned over to her parents at the crash screen.

On Monday May 10th, 2021 at approximately 0825 hours, dispatch sent units to the intersection of Eldredge Street and Vernon Street for a report of a road rage incident involving a crossing guard and a motor vehicle. When units arrived it was determined that the crossing guard who was performing her duties at the time of the incident, was struck by a motor vehicle that left the scene without stopping. I responded to the scene of the accident to conduct an investigation. At the time of the incident the weather was cloudy and the road surface was dry. Eldredge Street and Vernon Street are both public ways owned and maintained by the City of Newton. Upon arrival I spoke with the injured crossing guard inside the ambulance identified as, Margaret Monbouquette. Margaret was being treated by Fallon Medic 3 for leg pain. Margaret stated she was in the middle of the intersection holding her stop sign up as children and adults were actively crossing to get to the Underwood School. Margaret stated she had all three streets stopped when her attention was brought to the vehicle at the stop sign on Eldredge Street on the one way side. Margaret stated it kept inching forward into the intersection attempting to turn left. Margaret stated she still had people crossing at that intersection so she stepped in front of the vehicle and held up the stop sign directly in front of the vehicle and the female driver. Margaret stated she looked at the female driver and stated "No, stop" and that is when the driver started to yell back at her inside her vehicle but she could not make out what she was saying as her window was rolled up. Margaret stated the driver then tried to drive around her and struck her with the front passenger side bumper of the vehicle. She stated she was struck in her hip area which caused her to fall onto the street. Margaret stated the vehicle never stopped and sped up Vernon Street. Margaret described the driver as a slight female with dark hair driving a dark colored SUV. Margaret was able to provide a license plate of MA REG: 1XCS97. A query of the license plate through dispatch was able to provide an address of 74 Charlesbank Rd, Newton. Officers checked this address but got no response at the door and the vehicle was not located. A witness to the incident identified as Alicia Matos left her phone number and I was able to make contact with her over the phone. She stated she was in her motor vehicle stopped at the stop sign on Eldredge St, directly across from the vehicle involved. Alicia stated she was stopped because the crossing guard was in the intersection holding up her stop sign and was actively crossing children and adults. Alicia states she noticed the vehicle across from her kept creeping out into the intersection trying to take the left turn up Vernon Street. She states the crossing guard walked over to that vehicle and told her to stop and saw the female driver start yelling and pointing her hands at her inside the vehicle. Alicia states the crossing guard then backed up and was looking down at the drivers license plate when the driver drove at the crossing guard and struck her with her front passenger side and knocked the crossing guard to the ground. Alicia states the vehicle clearly made contact and did not stop and left the scene turning onto Vernon Street at a high rate of speed. Alicia states several people in the area came to the crossing guards aid and police and medics arrived shortly. Alicia described the driver of the motor vehicle to be a female with dark hair. The principal of the Underwood School, Suzie Talukdar was present after the accident and stated the secretary of the school received a phone call shortly after the incident from a grandparent of the school. The grandparent was identified as Dawn Coleman. Dawn stated her daughter was driving the vehicle involved and was having anxiety over the incident and didn't know what to do. Dawn identified the driver as Chelsey Sullivan (S83874401). I attempted to make contact with Chelsey at her residence of 74 Charlesbank Rd with negative results. The secretary was able to provide phone numbers for Dawn and Chelsey and I responded to the police station to attempt to make contact. While at the police station, I was notified that Chelsey Sullivan was downstairs in the lobby. I met with Chelsey and she stated the following: She was driving MA REG 1XCS97 a black Toyota Rav4 and was stopped at the stop sign on Eldredge Street the one way side. She noticed the crossing guard in the intersection holding up the stop sign but stated there was no one crossing at the time. Chelsey stated she started to inch forward and wanted to turn left onto Vernon Street as she was in a rush. Chelsey stated the crossing guard came right up to her vehicle and started shouting and holding up the stop sign in her hand telling her to stop. Chelsey stated she was yelling back at her stating that the cars behind her were going so she wanted to go too. Chelsey stated when the crossing guard started to walk away from her, she got very anxious and decided to take the left onto Vernon Street. She stated she saw the crossing guard stumble and fall to the ground but at no time did she think she made contact with the crossing guard. Chelsey stated she drove home and called her mother to tell her about the incident. Chelsey stated she wrote an apology letter to the crossing guard and has really bad anxiety and wasn't sure what to do. I advised Chelsey she needs to follow the crossing guards instructions when they are performing their duties no matter if she thinks the road is clear for her to enter/pass. Chelsey stated she understood and was also advised she will be getting a citation and summons in the mail. Chelsey took me to her vehicle where I took several photographs and submitted them to the IT Bureau. No damage was reported or observed. Massachusetts Uniform Citation 997066AA and criminal application was issued and mailed to Chelsey for Chapter 90 section 24 Operating to Endanger and Chapter 90 section 14 Failure to slow near pedestrian.

On Wednesday, May 19th 2021, at approximately 4:40pm, I, Officer Brooks, responded to 457 Walnut street, the Newton North High school, for a hit and run that occurred in the parking lot. I spoke with the operator of MV1(MA REG 672MT6), who stated his vehicle had been parked in the lot all day. When he came out he found a note on the car saying sorry that I backed into your car, and a phone number (617-899-4204). No other information was provided. MV1 had a plastic fascia piece broken off of the front bumper. Attempts to reach the other driver at the number provided were unsuccessful and a voicemail was left. The operator of MV1 was provided with the report number. There are cameras on the high school that face the parking lot and may have caught the accident, but I was not able to access them at this time. MV1 was parked in the 4th row of spots if you count them from the wall of the school out towards the street. Traffic Bureau update (Officer Gaudet): I was able to obtain a name associated with the phone number provided on the note of MV1. The phone number is registered to Mr. Dominic Matos (SA2100551). I attmepted to contact Mr. Matos with a negative result. I left a message for him to contact me to provide a statement.

Narrative:
On 06/14/21 at approximately 12:00 hours, Sgt. Wade received an email from Nina Wang (D.P.W. Transportation Traffic Superintendent) that a city utility trailer with a signboard attached to it was struck by another motor vehicle at 275 Brookline Street sometime over the weekend between 06/11/21 and 06/14/21. A photo was sent over in the email of the utility trailer and signboard knocked over on it's side over the sidewalk. The utility trailer is 2020 Wanco color orange bearing MA Official registration # M9994A. The trailer was parked in the bicycle lane of the northbound travel lane next to 275 Brookline Street. The signboard was there in an attempt to get motorists to drive the speed limit due to the Memorial Spalding School located at 250 Brookline Street. On 06/14/21 at approximately 12:00 hours, I responded to Brookline Street to photograph the trailer and possibly process the scene. The trailer could not be located. I learned later that the trailer had already been towed by Tody's towing and brought to the City D.P.W. yard located at 110 Crafts Street. On 06/15/21 at approximately 09:00 hours, I went over to 110 Crafts Street and was told that the trailer/signboard was in the garage. The trailer was already being repaired to be placed back in to service. It had sustained damage to the two rear brackets where the jacks are located and the license plate bracket and registration plate were bent. The signboard message frame board is no longer aligned properly as a result of the accident but reportedly still functions. After, I then responded back to 275 Brookline Street and took digital photos of the scene. I observed in the bike lane at the accident scene two small pieces of lens covers to a vehicle with no identifying marks. I also did a canvass for possible surveillance cameras in the area. There was no response at number 269 and 275 Brookline Street but I left a note for them to call me. 283 Brookline Street has no surveillance cameras. Memorial Spalding School is across the street from the accident and I observed a camera on the side of the building which faces the staff parking lot on to Brookline Street. I spoke to a staff member Jesse Brackett at the school. He called David Stickney (Superintendent of Schools) and notified him of the accident. Mr. Stickney was provided with my email and will send over any possible footage if he has it. At approximately 12:45 hours, I spoke to Mike Sylvester who resides at 275 Brookline Street. He stated that he had surveillance footage and emailed it over to me. I viewed the footage which is a camera that views his approx. 75 foot driveway leading out to Brookline Street. There is a fence and bushes behind where the signboard is located which limits it's view. The video shows the accident took place on 06/11/21 at 18:48 hours. At this time, you can see the trailer is struck and slowly falls over on it's side. Immediately after a white colored four door sedan continues to slowly drive past the scene northbound on Brookline Street in the bicycle lane without stopping. Two other vehicles behind the white sedan drive around to the left of the white vehicle at this point. In the footage, you can not view the registration number, operator details, or the type of sedan from the footage. On 06/16/21 at 09:40 hours, I spoke to David Stickney who called the Traffic Bureau. He stated that he had footage of the accident but it did not reveal any other details besides the same white sedan striking the trailer.

On Wednesday, June 16, 2021 while assigned to Traffic unit N525, I responded to the front entrance of the Brown Middle School located at 125 Meadowbrook Road in Newton for a single vehicle crash involving a City of Newton contracted school bus. It was also reported by Newton Dispatch that one of the occupants on the bus had suffered an arm injury as a result of the crash. Meadowbrook Road is a public way maintained by the City of Newton. The road surface at the time of the crash was dry. The weather at the time of the crash was clear and sunny. When I arrived, I was updated by Officer Iarossi that the crash involved a City of Newton contracted school bus transporting students from the Williams School (141 Grove Street, Newton). The students were participating in "Step Up" day and were leaving Brown to go back to Williams. Officer Iarossi stated a student on the bus was injured as a result of the crash. Tegan Quirk was transported by Fallon Medics to Newton Wellesley Hospital with a right arm injury. The remaining students that were on the bus reported no injuries to staff at the time and were moved to another bus to be transported back to the Williams School. I asked the Brown staff that was assembled in front of the school to contact the Williams School and ask them to hold the children involved in the crash at the school once they arrived. I made this request so we could make sure there weren't any new injuries reported; and I wanted to inquire if any of the children witnessed the incident. The staff stated they would contact Williams and relay the request. I spoke with the operator of the school bus (MA BUN: BU44536), Mr. John Aubert (S48613114). Mr. Aubert stated he was operating bus #361 while transporting students from Brown Middle School to the Williams School. Mr. Aubert stated as he was preparing to leave Brown, he made an announcement to students on the bus to sit down. Mr. Aubert stated he got the okay to leave from one of the staff members on the bus. Mr. Aubert stated there was another bus parked in front of him prior to leaving. Mr. Aubert stated he began to reverse the bus to create distance from the bus in front of him, and then operated the bus forward to exit from his position on the right side of the road and merge into the travel lane to his left around the bus parked in front of him. Mr. Aubert stated as he merged onto the roadway, he heard screaming from the students on the bus and the rear passenger side of the bus made contact with a brick support column in front of the school positioned along the sidewalks edge next to the roadway. Mr. Aubert stated he exited the bus and observed a student's arm pinned on the rear passenger side of the bus between the side of the bus and the brick column. Mr. Aubert stated he got back in the bus and reversed it until the students arm was free. Mr. Aubert stated at this time, staff helped the injured student. The staff there helped facilitate transportation for the remaining students back to the Williams School. Mr. Aubert reported no injuries, but was concerned for the welfare of the student. After speaking with Mr. Aubert, I observed damage to the school bus and the brick support column as a result of the crash. I observed fresh damage (scrapes) to the rear passenger side of the school bus between the last passenger side window and the back of the bus. I also observed fresh damage to the brick support column. The column is a support to an overhead covering that runs from the sidewalk to the front entrance of the school. There was fresh paint transfer from the bus and a crack in the column. Pictures were taken of the damage and submitted to the IT Bureau. The travel lane where the crash took place is a one way access road that runs from Meadowbrook Road to the front of the school, and exits onto a parking lot between the Brown Middle School and the Oak Hill Middle School. I responded to the Williams School to follow up with staff there regarding this crash. All of the students that were involved in the crash had been released prior to my arrival. I spoke with the Principal of the Williams School, Ms. Ayesha Farag, Ms. Michal Shapiro (Social Worker), and Ms. Katie Costello (School Nurse) about the students on the bus. They stated no injuries were reported to them, and they notified the parents of the involved parties about the crash. They also provided me with contact information for all of the students on the bus at the time of the crash. I spoke with the three teachers that were on the bus at the time of the crash, Ms. Kimberly Andersen, Ms. Tess McLean, and Ms. Katherine O'Leary. All three teachers stated prior to the bus leaving Brown Middle School they accounted for all of the students and were ready to leave to travel back to the Williams School. One of the teachers recalled hearing the bus driver make an announcement to the bus that they were about to leave, but could not recall the exact wording of the announcement. The teachers stated as the bus began to move, they heard screaming from the rear of the bus, looked back, and saw a student's arm stuck outside the window. After realizing the student was injured, they assisted with helping to free the student from his pinned position. They helped the injured student until EMS arrived on scene. They assisted the remaining students by directing them away from the crash and onto another bus for transportation back to Williams. The three teachers reported no injuries as a result of the crash. I responded to the Newton Wellesley Hospital Pediatric Emergency Room to check on the status of the injured student, Tegan Quirk. I spoke with Tegan and his mother, Kathryn Quirk in his room. While attempting to talk to Tegan and his mother, Tegan became very upset and did not want to talk about the specifics of the crash. Out of courtesy for Tegan, I exited the room and spoke with his mother outside the room. I explained to Ms. Quirk the events of the crash, and how her son's arm became injured. I asked Ms. Quirk if she could assist me with faciliating a conversation with Tegan about the events of the crash. I explained to her I understood how traumatic the events of the crash were for her son, but as part of my investigation I would need to find out what led to Tegan's arm being outside the bus window at the time of the crash. While speaking to his mother, Tegan explained that he was waving to a friend with his arm out of the bus window when the bus started to move. Tegan stated his arm hit the column, and became stuck between the bus and the column. Tegan stated he was screaming for the bus to move and that it took forever for the bus to move so he could free his arm. Tegan stated the bus driver gave no warning to the students that the bus was going to start moving. The staff at Newton Wellesley Hospital stated Tegan has a right arm injury. The severity of the injury is unknown at this time due to further examination that needs to take place. I advised Ms. Quirk about the conversations I had with the other parties involved. I spoke with Mr. David Stickney (Head of Facilities) from the Newton Public Schools to try and obtain footage of the crash. Mr. Stickney stated unfortunately there is not footage of the crash from the Brown Middle School or Oak Hill Middle School surveillance cameras. I advised Ms. Regina Moody (Newton Public Schools Transportation Director) about the crash. Ms. Moody was able to show me footage of the incident from the camera inside the bus. In the footage, the bus driver can be heard announcing to the students prior to the bus moving, "Guys, guys, get off the seat and sit correctly". One of the teachers can be seen doing a count of the students. The teacher is heard saying to the student, "Everyone needs to be in a seat and sitting at 3 and 2 and 1 and thank you." Student are observed still standing up. The teacher is then heard saying, "sit down sit down." After the teacher passes the students in the back of the bus, she says, "We are good." to the front of the bus. Students are then observed standing after she passes. As the bus begins to back up, students can be observed standing in the rear of the bus and not sitting down on their seats. As the bus moves forward, a student (Tegan Quirk) can be seen standing on his seat and extending his right arm out of the last window on the passenger side of the bus. Screaming can then be heard as his arm makes contact with a support column on the passenger side of the bus. The bus is stopped, the driver exits the bus to asses what happend, gets back on the bus, and reverses the bus a short distance. The injured students right arm is released and he brings his arm back into the bus through the window. The amount of time between the students arm being pinned against the column to when it is released is approximately 40 seconds. After speaking with all parties involved and reviewing the footage from inside the bus, it appears that Bus #361 was pulling out of it's parked position in front of the Brown Middle School to enter the roadway and return students to the Williams School. A student that was sitting in the last row on the passenger side of the bus extended his right arm out of a window next to his seat to say goodbye to a friend. While his right arm was extended out of the rear passenger side window of the bus , the bus collided with a brick support column affixed to the sidewalk in front of the school. When the bus made contact with the column, the students right arm made contact with a brick support column and became pinned between the column and the passegner side of the bus. After the students arm was freed from it's pinned position, the student was able to bring his arm back into the school bus and was transported to Newton Wellesley Hospital for a right arm injury.

On 06/22/21 at approximately 10:46 I responded to a 2 vehicle crash at the intersection of Winchester St. and Dedham St.. Upon arrival I spoke with the operator of veh #1 Giulia Puppin who has her learners Permit and was driving under the CS Driving School LLC and Instructor John David Rabidou , who was in the front seat next to her. Puppin explained that she was driving South down Winchester St. when she put her left directional on to take a left onto Dedham St.. Puppin said the traffic light turned yellow so she stopped the vehicle before the intersection and thats when veh #2 struck her from behind. Rabidou's account of the accident was the same as Puppin's. I spoke with the operator/owner of veh #2, Diana Gabriela Valencia who stated she was driving South down Winchester St. when veh #1 stopped suddenly in front of her at the intersection of Dedham St.. She attempted to swerve right to avoid the vehicle but she didn't have time and struck the passenger side rear bumper. As I was in my cruiser preparing the exchange information, Valencia came to my window and informed me that her vehicle was unregistered. A R.M.V. query confirmed that Massachusetts license plate attached to the front and rear of the vehicle she was operating, a black 2009 Hyundai Accent , was active on a Grey 2013 Volkswagon Jetta owned by her boyfriend Frederic James Malesevic. Valencia produced a bill of sale for the Hyundai Accent from Vigeant's Auto Sales, INC of Lowell with a purchase date of 10/17/20. Valencia stated she never drove the vehicle before today and was in the registration process through the mail to the R.M.V.. Valencia was handed Mass. Criminal citations #'s T20808053 and T20808054 for : Ch 90/ Sec 23-Attaching wrong M.V. plates on a public way Ch 90/ Sec 34J- Operating a M.V. without insurance on a public way Ch 90/ Sec 9- Unregistered M.V. on a public way N.C.O. 19/75- Failure to use care in stopping Both front and rear license plates (Mass.#5ZS55 were confiscated from the vehicle and placed into our Property and Evidence until the matter is resolved in court. All roads mentioned/traveled upon in this report are public way in the City Of Newton as they are serviced and maintained by this city. Incident report #21026716 and a towed M.V. Form were also completed.

Narrative:
On 07/28/21 at 13:39 hours, I (Traffic Unit # 523) responded to 10 Cummings Road for a past hit and run motor vehicle bicyclist crash which occurred approximately twenty minutes prior. Dispatch updated units that the accident occurred at the intersection of Watertown and Walnut Streets and that a white colored SUV left the scene after striking the bicyclist. The white SUV was last seen heading southbound on Walnut Street towards Washington Street. The bicyclist then walked back home to 10 Cummings Road and told her mother about the incident. The mother then notified NPD. Watertown Street and Walnut Street are both public ways maintained by the City of Newton. It is a four way intersection with crosswalks on every side. There are no turn on red signs on every corner of the intersection. The weather at the time was partly cloudy and sunny and the road conditions were dry. On arrival at 10 Cummings Road, I spoke to the mother identified as Vanessa Cruz and her daughter, Angelique Cruz, who was involved in the crash. Fallon Ambulance was already on scene and a patient refusal was signed as Angelique did not have any injuries. I asked Angelique what had occurred and she stated that she left FA Day Middle School at approximately 13:00 hours. It is estimated that the crash took place between 13:10 and 13:20 hours. She was then bicycling down to the intersection of Walnut & Watertown Streets. She says that she pushed the walk signal button to cross the street at the crosswalk to continue southbound on Walnut Street towards Lowell Avenue. Once the cross signal light activated, she bicycled through the crosswalk. At the tail end of the crosswalk, her rear bicycle tire on the right side was struck by a white SUV making a right turn on to Walnut Street from Watertown Street. She was then knocked to the ground where she sustained a scrape mark to her left knee on her sweat pants. The white SUV continued southbound on to Walnut Street without stopping. She feels that the driver might of misjudged taking the right turn and never realized that she struck her. Angelique says she only saw a female operator in the SUV and could not determine the race or age. She also did not observe the registration on the vehicle. She further stated that a female assisted her up from the ground. Once she said she was fine she then proceeded home. No one else phoned 911 regarding the incident. At this point, I asked to see the bicycle which was stored away. The bicycle is a Schwinn Sidewinder 21 speed color black with lime green block style writing for the brand and model on it. Both tires have white reflectors attached to them. I observed no damage to the bicycle. At the time of the crash, Angelique stated she was wearing purple sweat pants, blue sweater, and no bicycle helmet. She was also carrying a black backpack attached to her rear shoulders. I took digital photos of Angelique along with her Schwinn bicycle. After, I went over to the intersection of Watertown and Walnut Streets and took photos of the scene. A canvass was also conducted that produced negative results. I observed no surveillance cameras on the buildings or residences that are near the intersection. At 109 Walnut Street, I spoke to several tenants in the six unit apartment complex and they have no security cameras. 583 Watertown Street has no cameras along with 592 Watertown Street. I also checked near the intersection of Walnut and Washington Streets as the white SUV was last seen heading in that direction. The Trio apartment complex along with other businesses are lined against both streets and have outside security cameras. I spoke to the leasing office and they stated that the cameras do not provide any coverage of the street area.

Upon arrival, I observed vehicle 1 parked on Richards Circle with minor damage to the drivers side mirror. Operator 1, Matthew Kelly, stated he was parked by the curb on Crafts St (public way in the City of Newton ) across from F.A. Day Middle School and when he entered his vehicle and closed the driver's side door, vehicle 2 side swiped his vehicle causing damage to his mirror. He then saw Massachusetts Registration: 1GRH91, a White 2012 Nissan Rouge, continue northbound on Crafts St and not stop. Operator 1 stated he followed vehicle 2 on Crafts St until he got to the Waltham line on Waltham St. Operator 1 then pulled into Richards Circle and called dispatch to report the accident. Operator 1 stated he could not tell if Operator 2 was a male or female, but stated there was damage to the passenger side mirror of vehicle 2. The registration for vehicle 2, comes back to a Walter and Analise Wilson out of Waltham. Waltham PD responded to both of their addresses to attempt to raise them with negative results. There was also not a phone number listed for either party. There was no injuries reported and no tows were needed. Traffic Bureau update (Officer Gaudet): I attempted to contact both registered owners for MV2 with a negative result. A "Hit and Run" inquiry was mailed to Mr. Walter Wilson and Ms. Analise Wilson regarding their possible involvement in this crash.

On Wednesday, September 1, 2021, while assigned to marked unit N525, I responded to the area of 1488 Washington Street, Newton for a report of a motor vehicle crash involving a school bus. The weather at the time of the crash was cloudy was light rain. The road surface was wet. Washington Street is a public way maintained by the City of Newton. The operator of MV1, Fanes Nerestant (S47503048) stated he was operating a City of Boston owned 2012 Chevrolet School Bus (MA SBN: SB37810) west on Washington Street towards the Learning Prep. Mr. Nerestant stated he slowed his vehicle down and put on his right direction to take a right turn into the parking lot at 1488 Washington Street to pick up students at the Learning Prep School. Mr. Nerestant stated he then felt impact from the back of the bus and heard a "boom". Mr. Nerestant was transported to Newton Wellesley Hospital for neck and back pain. I observed minor damage to the rear driver side bumper/fender area. There were no other occupants on the bus at the time of the crash. The operator of MV2, Mr. Allen Simon (S44212695) stated he was operating his 2018 Nissan Altima (MA: CS12BM) west on Washington Street near 1488 Washington Street. Mr. Simon stated the school bus in front of him in his travel lane abruptly stopped to make a turn and he was unable to avoid crashing into the vehicle. MV2's front passenger side front end sustained heavy damage as a result of the collison. Mr. Simon reported no injuries. MV2 was removed from the roadway by Tody's towing. Mr. Simon stated his vehicle had a camera in it at the time of the crash and Mr. Simon stated he would forward the footage of the crash to me.

The owner of MV#1 called the NPD to report that she parked her MV in the rear parking lot of the Franklin School, 125 Derby St on 9/2/21 at 10AM and when she returned to her MV at 3PM it had damage to the rear drivers side. I spoke with the custodians at the Franklin School and they state that they were aware of the MV in question because it was parked in an area where it wasn't supposed to be, but otherwise had no information on provide. There are exterior cameras at the rear of the school that are working and I have contacted the appropriate person for footage. To be further investigated.

Narrative:
On Tuesday, September 21, 2021, I received a call from Sgt. Mike Wade of the Newton Police Traffic Bureau. Sgt. Wade stated a serious MVA/Bicycle crash had just occurred at the intersection of Cherry Street and Derby Street in Newton and the bicyclist was a 10-year-old boy with a possible head injury. Sgt. Wade stated the Traffic Bureau was requested to respond by the Shift Commander and asked if I could respond to investigate the crash. On arrival, Officers reported to me that the cyclist involved, Benjamin Lavoie, was transported to Children's Hospital in Boston with a possible head injury. It was reported to them that Benjamin was traveling Southbound on Cherry Street towards Derby Street on his bicycle. Benjamin entered the intersection of Derby Street and crashed into the front passenger side area of a passing motorist that had a green light. Officers on the scene also reported that the motor vehicle and the bicycle involved had been moved from their original rest location. Cherry Street is a public roadway maintained by the City of Newton. It supports two way travel (N and S) from Washington Street (S) to Waltham Street (N). The travel lanes are divided by an unprotected double yellow line median. The posted speed limit for this street is 25 miles per hour. Derby Street is a public roadway maintained by the City of Newton. It supports two way travel (E and W) from Waltham Street (E) to Lexington Street (W). The travel lanes are divided by an unprotected double yellow line median. The posted speed limit for this street is 25 miles per hour. The weather at the time of the crash was clear and sunny. The road surface in the immediate area of the crash was dry. I spoke with the operator of MV1, Ms. Erica Pusateri (S61409866). Ms. Pusateri stated he was operating her 2021 Kia Seltos (MA: 8HC743) Westbound on Derby Street towards Cherry Street. Ms. Pusateri stated that she had a green light at the intersection and proceeded to cross Cherry Street to continue onto Derby Street. Ms. Pusateri stated halfway through the intersection she heard a noise and saw a bicyclist hit the front passenger side door area of her vehicle. The collision caused the glass window from the front driver-side door to shatter. Ms. Pusateri's vehicle controlled her vehicle to a stop and she observed a young boy lying on the roadway in the intersection behind her. Ms. Pusateri exited her vehicle, ran over to the injured bicyclist, and rendered aid to him by stabilizing his neck. Ms. Pusateri remained on scene and cooperated fully with Officers. I asked Ms. Pusateri if she was distracted at all while operating her vehicle prior to the collison. Ms. Pusateri stated she was not. I asked Ms. Pusateri if she could recall where in the intersection the bicyclist crashed into her vehicle. Ms. Pusateri stated the bicyclist crashed into her vehicle in the Northwest side of Derby Street just outside of the marked crosswalk in the intersection. Ms. Pusateri stated she knows it was in the street in the intersection because she observed a woman with a stroller on the sidewalk on the corner of Cherry Street at Derby Street on the Franklin Elementary School side and she had already travelled through the intersection passed the East side of Cherry Street. Due to the unknown injury status of the bicyclist, Ms. Pusateri's vehicle was towed to the Newton Police Station and secured in the garage area as evidence. Ms. Pusateri reported a small laceration on her right hand with no other injuries. I observed damage to the front passenger side fender/door area. There was no airbag deployment as a result of the crash. I observed glass through out the front passenger side compartment area of the vehicle from the window breaking. I contacted Benjamin' s parents, Cheryl and John, to check on the status of their son. Cheryl stated Benjamin did not suffer any life-threatening injuries and appears to have bumps, bruises, and a cut on his head. I asked if I could speak with Benjamin for his account of what happened at the crash. Benjamin stated he was riding his bicycle on Cherry Street (Southbound) on the sidewalk on the East side of Cherry Street. Benjamin stated he left the sidewalk and entered the intersection to cross Derby Street and continue on Cherry Street. Benjamin acknowledged that at this time Cherry Street had a red light and Derby Street had a green light. Benjamin stated he then crashed into a passing car in the roadway. Benjamin was wearing a helmet at the time of the crash. The helmet had significant damage to it as a result of the collision. I told Benjamin his bicycle was being held at the Newton Police Department for safekeeping and we will arrange to get it back to him. Benjamin is a student at the Franklin Elementary School. I make contact with a witness to the crash, Mr. Benjamin Crosby. Mr. Crosby stated he did not witness the actual crash, just the aftermath. I was able to reviewthe 9-1-1 call Mr. Crosby made regarding the crash in Newton Disptach. Mr. Crosby stated a boy on a bicycle was in a crash with a motor vehicle. Mr. Crosby is heard saying "he went through the light" to the Dispatcher taking the call. After completing my investigation, I have summized the following. The bicyclist (Benjamin Lavoie) traveled down Cherry Street (S) and went through a red light to cross the intersection at Derby Street (W). As a result of traveling through the red light and entering the intersection, he crashed into the front passenger side door area of a passing motorist that had a green light (Erica Pusateri). No citation will be issued as a result of this crash.

On 09/27/2021, while assigned to N503, I, Officer Conary, responded to the area of Pettee Street, Emerson Playground, for a minor MVA involving a school bus. Upon arrival, I met with Operator of MV1 who explained to me that MV2 (school bus) had to leave and continue their bus route. Operator of MV1 was able to provide me with the drivers information. Newton Dispatch also called Eastern Bus to confirm the information. Operator of MV1 was parked Northbound on the left side of the street when MV1 hit her rear left quarter panel. MV2 was pulling off to the side by the park to let students out. Operator of MV1 stated she saw student on the bus and got off. She said there was approximately a handful of students (6 or 7). She believes that they were from the Zervas School on Beacon Street. MV2 was the third bus that pulled up at the park to drop students off. I attempted to call Operator of MV1 with negative results but left a message. I tried to call Eastern Bus Company again with negative results. An email was sent to Regina Moody, City of Newton, who oversees the school buses. The traffic bureau has been notified for follow up. SRO of the Zervas School was notified as well. MV1 was able to be driven from scene, and no injuries were reported. On 10/26/21, I followed up with Regina Mooney with the City of Newton and she was able to comfirm that there were a few students on the bus at the time of the accident. Eastern Bus Company is handling the accident.

On 10/19/21 at 08:30 hours, Sgt. Wade notified me that he received an email regarding a motor vehicle accident involving a pedestrian/student that occurred on 10/15/21 between the hours of 08:20am and 08:35am. The crash was never reported by the operator of the vehicle or the student/pedestrian. Approximately two hours later, the student went to the nurse's office seeking assistance for neck pain which is how the school became aware. This crash occurred in the marked crosswalk on Brandeis Road in front of Newton South High School which is located at 139 Brandeis Road. The weather at the time was 63 degrees with mostly cloudy and sunny conditions (wunderground.com). Brandeis Road is a two way roadway divided by an unprotected median that runs east and westbound. In front of the school there is a marked crosswalk in the roadway. There is also a one way driveway near the main entrance of the school with parking stalls and parents can drop their children off there on the right side of the driveway. The driveway then leads back out to Brandeis Road. Right after you exit the lot there is the above mentioned crosswalk to the left of the driveway which is very visible with nothing obstructing your view. On arrival, I spoke to the Dean of the school named Charlie Myette. Mr. Myette brought me to his office and started looking at the school's video surveillance system. He also requested that the student/pedestrian involved to report to his office. The student was identified as Tsukasa Iinishi. I asked Mr. Iinishi what had occurred and he stated that he was walking to school on the morning of 10/15/21. He says that he was wearing a pink hoody sweatshirt with gray shorts. He was walking through the woods which is across the street from the school which exit out to Brandeis Road. He says that he waited at the crosswalk and let two vehicles go westbound by him. He then thought it was clear to proceed and started walking. At the tail end of the crosswalk he was struck by a red colored Toyota Prius which exited left from the driveway and was on the left side of the roadway. He was struck on the left side of his body and he says that he used his left hand to brace himself against the hood and that's when the Prius stopped. He says that the operator of the Prius asked him if he was okay and he replied that he was. He then kept walking toward the school. The operator then continued on it's way without providing any information or notifying us. He described the operator as an Asian female with black hair approximately 40 to 50 yrs in age. During this time, Mr. Myette was able to find surveillance video of the crash. There are two camera angles. One shows the driveway along with the walkway of the school building. The other one shows the same but it also overlooks out in to Brandeis Road. The view of the road is partially blocked from the parked cars in the lot. The video shows a red colored Toyota Prius pull in to the driveway of the school. An Asian female student wearing a white long sleeve shirt with green lettering and dark colored jeans exits the passenger front of the Prius and eventually enters the school. The Prius then pulls out and goes toward the left side of the driveway around other vehicles waiting to exit. At 08:27 hours, the Prius pulls out angling towards the left side of the driveway going around other vehicles waiting to exit. The Prius then exits the lot on the left side (wrong side) of the roadway and makes contact with the pedestrian. In the video you cannot see the front hood area of the car, but you see Iinishi's torso bend towards the vehicle on impact. It shows the Prius stopped there for approximately ten seconds and then it drives away. In the video you are not able to see the registration plate number. Mr. Myette is going to send the photo of the student dropped off in the Prius to his staff to have her identified so I can follow up with the operator. On 10/21/21, I received an email from Mr. Myette stating that some of his staff has not come forward on the identification of the student. He stated that he would now send a message out to a larger group of staff to see if he gets a response. He also attempted to enhance the registration plate using the surveillance software but the plate is still too blurry to make out. On 10/25/21 at approximately 08:40 hours, I went over to Newton South High School to see if I could locate the red Toyota Prius involved in the crash. As I was driving on Olde Field Road approaching Brandeis Road near the school, I observed a red Prius approximately four cars ahead of me in heavy school traffic. Eventually, I observed the Prius pull in to the drop off driveway of the school and drop an Asian female student off. I identified her as the same student that got out of Prius prior to the past accident on 10/15/21. The operator was an Asian female which appeared to be in her late forties. After, the Prius exits the lot on to Brandeis Road and I flagged her down. I had her pull in to the parking lot across the street from the school. I asked her if she was involved in a accident with a student on the morning of 10/15/21 to which she replied that she was. She was identified as Xiaojin Liu through her MA Driver's License # S49033927. She was operating a 2010 Toyota Prius bearing MA reg.# 8JY630. I asked her what occurred and she stated that she was exiting the driveway of the school to turn left on to Brandeis Road. She says that she has a habit to think that no one is ever walking through the crosswalk and thought it was clear to proceed. She turned and then saw the student crossing and attempted to stop but was not able to in time. She then bumped the student with the front of her vehicle. She says that she stopped and asked the student if he was ok several times and he stated that he was. The student then walked away and she says that she did not know what else to do. I advised her that she should of gave her information to the student, notify the school, or call us to which she understood. I issued MA Uniform Citation # 172129AB to Ms. Liu for Ch. 90/24/A Leaving the scene of personal injury. After, I went inside the school and notified Mr. Myette of my findings. I asked him for the surveillance footage as I previously did in person and through email. He stated that he would put the video on a zip drive and notify me.

On 10/22/2021, at approx 0752Hrs, while assigned to 497, I responded to 2071 Washington St for a report of a past hit and run with property damage. Upon arrival I observed the tire tracks leading from the right westbound lane leading over the curb , knocking over a chain link fence and post . There were tire ruts into the side yard of 2071 Washington St, an there was a End of Reduced Salt sign and post wedged into the ground of the homeowners side yard. I spoke with the property owner , Ali Shamser who stated at approx midnight he heard a loud bang, and looked out his window , saw nothing in the road but did not think to look down into his side yard. Ali's home is across the street from Newton Wellesley Hospital and next to Route 128 as well as on a very busy road so he is used to hearing crashes and sirens regularly. Ali stated at approx 0745Hrs when he left his home to take his children to school he noticed his damaged fence and yard. Ali has no cameras, across the street is Newton Wellesley Hospital and his neighbor adjacent on Walsingham St has his property surrounded by a 6 foot vinyl fence.

MV#1 was parked and unoccupied at the rear of the Franklin School, #125 Derby St on 10/18/21. According to school staff, sometime around 12:30PM another MV that was parked in the rear lot attempted to back out of their parking spot and struck MV#1. School staff states the crash is on video and they are trying to determine the identity of the operator. The operator of MV#2 is believed to be a visiting nurse. Request this hit & run to be followed up by the Traffic Bureau. Traffic Bureau update (Officer Gaudet): On Tuesday, October 26, 2021 I made contact with the Franklin School to follow up with this crash. The staff there stated they have surmised the operator that left the scene with out exchanging information was a Covid tester that has been assisting with elementary schools in the city. The only information they had was that his name was Alan. The staff stated they were in the process of figuring out what school Alan was going to be at today and they would provide him with my contact information. A short time later, I received a phone call from a party named Alan Chua. Mr. Chua stated he was the operator involved in the crash on 10/22/21 at the Franklin Elementary School. Mr. Chua stated he was backing out of a spot in the parking lot behind the school when the rear of his vehicle crashed into MV1, which was parked and unoccupied in the lot. Mr. Chua stated he had an engagement he had to go to so he left the area and did not stop to exchange information or attempt to find the owner of MV1. Mr. Chua stated he is not sure if there is any damage to his vehicle as a result of the crash. Mr. Chua reported no injuries as a result of the crash. Mr. Chua was advised and will be mailed Massachusetts Uniform Citation 173173AB for Chapter 90, Section 24 (Leaving the Scene of Property Damage) and Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care).

On October 25th, 2021 at approximately 08:32 hours while working N491 I responded to a report of a MV crash in the area of Walnut St near Minot Place. On my arrival I located both involved vehicles near #100 Walnut St, The School Administration Building. Vehicle #1 was a blue 2015 Prius, Ma reg.945-AM3, operated by a William Aldridge. He stated he was first stopped N/B at the intersection of Walnut St @ Watertown St in the right turn only lane. The light turned green and realized he wanted to go straight so he continued heading N/B on Walnut St . However as he went straight he cut off vehicle #2 who was in the lane to his left who then honked his horn at him. Vehicle #1 then stopped ahead of vehicle #2 a short distance down the road. Vehicle #2 then drove around vehicle #1 who was still stopped. Vehicle #2 after going around vehicle #1 had to stop almost immediately due to a pedestrian crossing the street. Vehicle #1 at the same time proceeded forward behind vehicle #2 but then rear ended vehicle #2 after it stopped for the pedestrian. Vehicle #2 was a 2021 grey Toyota Highlander, Ma reg. 447AB8, operated by a Rizwan Khan. He stated he was stopped for the light N/B on Walnut St @ the intersection of Watertown St. He was in the lane for straight ahead traffic. As the light turned green vehicle #1 cut over into his lane ahead of him causing him to almost hit him. He then admitted beeping his horn at vehicle #1. Thats when vehicle #1 suddenly stopped in front of him. After vehicle #1 didn't move he decided to go around him. As he passed him there was a pedestrian crossing the street. He then stopped his vehicle so the person could cross. His vehicle was then rear ended by vehicle #1.

On October 26th 2021 while assigned to the front desk, Hanna Yoon DOB 9/9/83 came in to report a minor past mva/pedestrian. Yoon states that around 0820 hrs today, she was pulling out her driveway at 357 Langley Rd (Ma reg 5HL884). Yoon did not see a pedestrian approach from the right, and contact was made at a slow rate of speed. Yoon states the pedestrian did not fall down. She believes that the front right part of the vehicle hit the person's left thigh. Yoon asked the pedestrian if she was ok, to which the woman stated she was fine. The woman asked for and was provided Yoon's cell phone number before walking away. Her clothing description is unknown. Around 1100hrs Yoon then received a text from 617-913-5141. The person texting identified them self as the woman who was hit, and stated she was in the hospital with leg pain. She then texted that the hospital would contact Yoon for more information. Yoon did not respond and has not been contacted by anyone since this text. A name was not provided by the texter. There are no entries in QED indicating that the unknown pedestrian has called to report this accident. I tried to call the number provided with negative results. At this time, there is no identifiable victim. Yoon was advised that I will document her statement, and in the future it is best to call police to the scene of a crash in order to avoid confusion. Update by Lt. Dangelo: On 10/27/21 at approximately 0830 hrs. the pedestrian from the accident, Woo Jin Seo came into HQ with her mother to report the accident. She stated that on 10/26/21 at approximately 0820 hrs. she was walking to Newton South High School from 383 Langley rd. when a vehicle backing out of a nearby driveway bumped her. She stated that she did not fall down, but did suffer an injury to her leg. She stated that the operator of the vehicle stopped and checked and left her phone number. Ms. Seo went to Beth Israel Lahey urgent care the same day for leg and shoulder pain. I contacted the operator Hanna Yoon and updated her.

On 11/03/21 at 13:08 hours, Retired Detail Officer Edrehi reported that there was a motor vehicle accident on Parker Street at Boylston Street involving a city transport school bus. Parker Street is a public way maintained and owned by the City of Newton. The weather at the time was sunny with some clouds and the road conditions were dry. On arrival, both vehicles were moved away from the crash location and off to the side of the Boylston Street (EB) on ramp in order to relieve traffic congestion. I spoke to the operator of the school bus ( Vehicle # 2) identified as Jean Isme (MA D/L# S05201694). Mr. Isme works for the Eastern Bus Company which is contracted to transport children for the City of Newton public schools. At the time, he was operating a yellow 2022 Thomas School Bus bearing MA reg.# 1A6DJ. He states that he was traveling on the off ramp of Boylston Street (WB) at Parker Street and turned left on to Parker St. He was then trying to take a left turn on to the Boylston Street (EB) on ramp but could not due to traffic congestion. At this point, he was rear ended by Vehicle # 1. I observed no dents or paint damage to the rear of the school bus. I then spoke to the operator of Vehicle # 1 identified as Madeline Fordyce (MA D/L # SA3410026) who was operating a 2011 Chrysler 200 gray bearing MA reg. # 95S120 at the time. Ms. Fordyce stated she was behind the school bus coming off the westbound off ramp and was also making a left turn on to Parker Street. As she made the turn she noticed that the bus had stopped and attempted to brake, but she could not stop in time. The Chrysler sustained damage to the entire front bumper and the hood was pushed up. The vehicle was towed by Tody's towing. No injuries reported and digital photos were taken of the scene.

On Monday November, 15, 2021 at approximately 16:36hrs I, Officer Daly, responded to 303 Cabot Street for a past hit and run. Upon arrival, I spoke with the vehicle owner, Joanne Murphy. Murphy said she had her car parked in the neighbors driveway. Murphy said there were numerous large white vans parked at the house throughout the due to work that was being done. Murphy said she went to pick up the children from school at approximately 14:45hrs and returned to the house at approximately 15:00hrs. Murphy said she noticed damage to the rear driver side bumper near the wheel well. It should be noted there is a sizeable dent with white paint marks. Murphy did not witness another vehicle strike her vehicle. Murphy and her neighbors are unaware of the name of the company who is doing the work at the house. I advised Murphy to contact the landlord to see if they can locate the name of the company who is doing the work at the house. I canvassed the area for cameras yielding negative results. There were no injuries and Murphy's vehicle is able to drive without any issues. At 19:30hrs Murphy came into the Newton Police Station to provide me with the other driver's information. Murphy said she was contacted by Rostyslav Bulat, who said he was the driver who hit her vehicle. Bulat provided Murphy with all of his information. I called Bulat and asked him what had transpired. Bulat said he was not aware that he had struck Murphy's vehicle and was notified by his boss that Murphy's vehicle was struck as Bulat backed out of the driveway. Bulat said once he found out he struck the vehicle he got Murphy's information and provided her with all of his information. I advised Bulat that he has to be careful driving and if he hits a vehicle he must stop to provide his information. Bulat was apologetic and said he would be in contact with Murphy throughout the insurance process to ensure it gets done. It should be noted that Bulat has an active license from Ukraine. Bulat has lived in Massachusetts for approximately 2 months and has been in contact with the RMV to get a Massachusetts's license. Bulat said due to COVID the RMV has been delayed but Bulat has paperwork from the RMV showing his attempts to get an active Massachusetts's license.

On Tuesday, November 23, 2021, while assigned to Traffic unit N525, I was asked to investigate a past hit and run crash involving a City of Newton School Traffic Supervisor. Mr. Mark Dumoulin reported to the Newton Police Traffic Bureau that sometime during his assigned school crossing post on the morning of November 23, 2021, a passing vehicle made contact with his left leg while he was crossing children on Ward Street. I responded to Newton Wellesley Hospital to speak with Mr. Dumoulin. Mr. Dumoulin stated this morning, he was at his assigned school crossing post of Ward Street and Waverly Avenue in his official capacity as a City of Newton School Traffic Supervisor. Mr. Dumoulin stated he was wearing his department issued yellow reflective jacket and his department issued yellow hi-vis gloves. Mr. Dumoulin stated he also had his department issued stop sign with him while performing his duties. Mr. Dumoulin stated at some point during his post (0745-0830 hours), he was attempting to cross two children on Ward Street (S to N). Mr. Dumoulin stated he entered the crosswalk of Ward Street on the NE corner of Ward Street at Waverly Avenue. Mr. Dumoulin stated the pedestrian signal was activated at this time and the intersection traffic signals were all red. Mr. Dumoulin stated while entering the crosswalk, he observed a black Honda sedan traveling on Stuart Road (E) towards the intersection of Ward Street and Waverly Avenue. Mr. Dumoulin stated the license plate had 3 characters on it and believes the first letter may have been "E". Mr. Dumoulin stated the operator was a heavy set female with blonde hair in her 30's or 40's. Mr. Dumoulin stated the operator appeared to speed up towards his location to beat the light turning red. Mr. Dumoulin stated the vehicle then continued through the intersection onto Hammond Street (E), but it did not keep right with the normal flow of traffic and entered the Westbound lane of Hammond Street. Mr. Dumoulin stated he attempted to get the operators attention to stop her from continuing in the wrong lane of Hammond Street. Mr. Dumoulin stated at this time, the vehicle made contact with his left knee area and continued traveling down Hammond Street with out stopping. Mr. Dumoulin stated the children he was crossing were not near the vehicle and they did not see the contact. I asked Mr. Dumoulin if there were any other witnesses in the area. Mr. Dumoulin stated he does not believe there were. Mr. Dumoulin reported this collision to the Newton Police Traffic Bureau at approximately 1030 hours. I asked Mr. Dumoulin why he did not contact us when the incident first took place. Mr. Dumoulin stated he thought his injury was superficial and that he knew we were busy. At this time, Mr. Dumoulin is reporting a left knee injury as a result of the crash. Mr. Dumoulin stated vehicles travel on the wrong side of the medium on Hammond Street all the time and estimates it happens 3 times every school post. Mr. Dumoulin also stated the keep right sign on Hammond Street near the crash site needs to be addressed because some of the letters are peeling. I canvassed the area where the crash occurred for the fleeing vehicle with a negative result. I also covered the afternoon school post (1450-1535 hours) to ask passing students if they witnessed the crash with a negative result. I observed the "Keep Right" sign Mr. Dumoulin made mention of at this location. The sign appears to be in working condition and visible from Stuart Road. I took photos of the intersection where the crash happened and submitted them to the IT Bureau. On November 24, 2021, I covered Mr. Duboulin's school crossing post again for the morning post. While canvassing the area I observed a dark colored Mercedez sedan with a 3 character registration parked on Exmoor Street that resembled the vehicle Mr. Duboulin described as the fleeing vehicle. I attempted to contact the registered owner of this vehicle, Ms. Judtich Zukor. Ms. Zukor stated she could not speak at this moment and asked me to call her back. I will follow up with Ms. Zukor.

On 12/13/21 at 11:04 hours, I responded to the Mount Ida Campus of UMASS Amherst located at 777 Dedham Street for a one car crash involving a City of Newton Department of Public Works vehicle. The crash occurred at a y type walkway/driveway near the Shaw Building dining hall. The weather at the time was clear and approximately 50 degrees and the road conditions were dry. On arrival, I spoke to the operator of the DPW vehicle identified as William Bertrand (MA D/L # S24973075 ). Bertrand was operating a 2016 Ford Escape color black bearing MA Municipal registration # M2534A at the time of the crash. He states that he was at the Shaw campus building to conduct an inspection and was parked at a y type walkway/driveway outside of the building. When leaving, he attempted to make a three point turn to exit the driveway. While backing, he states that his foot slipped transitioning from the gas to the brake and he struck a light pole near the edge of the driveway. He says the back up sensors never alerted him of the pole either. The Ford Escape sustained minor scrape marks to the driver's side rear bumper area. As a result of the impact, the light pole cracked at the base and was knocked over on to the ground. On Thursday, January 20, 2022, while assigned to Traffic unit N525, I responded to the area of 1446 Beacon Street, Newton for a report of an MVA involving a City of Newton contracted school bus. The weather at the time of the crash was light rain. The road surface was wet. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1, Mr. Ralph Ghera (S43446773). Mr. Ghera stated he is a driver for JSC Transportation (contracted by the City of Newton) and was picking up a student to transport them to Newton South High School. Mr. Ghera stated his vehicle, a 2019 Ford Transit Van (MA SPN: 109403 ), was parked in front of Beacon Street (E) with it's red flashing lights activated while Ms. Ella Glennon was entering the vehicle for transport. At this time, MV2 (2020 Lincoln Aviator (MA: 3AB494) travelled past MV1 Eastbound on Beacon Street and disregarded the flashing lights that were activated on the van. MV2's front passenger side door mirror made contact with MV1's front driver side door mirror. I observed very minor damage to the mirrors on both vehicles. No injuries were reported. I spoke with Ella's father, Ian Glennon, on scene and made him aware of his daughters involvement in the crash. Mr. Glennon stated his daughter was not injured. Ella was then transported to Newton South by Mr. Ghera. The operator of MV2, Mr. Yipei Zhang (S13848781), was issued Massachusetts Uniform Citation 265793AB in hand for Chapter 90, Section 14 (Fail to stop for a school bus). Photos were taken and submitted to the IT Bureau.

On Monday, January 24, 2022, while assigned to Traffic unit N525, I responded to the Brown Middle School for a report of a crash involving a City of Newton contracted school bus. The school bus was involved in a crash on the Boylston Street (Rt.9) Eastbound offramp at Parker Street. Boylston Street and Parker Street are both public ways in the City of Newton. The weather at the time of the crash was clear and sunny. The road Surface was dry. I spoke with Officer Durickas of the Newton Police Department in front of Brown Middle School. Officer Durickas stated he witnessed a City of Newton contracted school bus sideswipe a National Grid contractor's vehicle on the Boylston Street (E) offramp just before Parker Street. Officer Durickas stated the school bus was filled with students headed to the Brown Middle School. Officer Durickas collected information for the owner of the vehicle that was hit and escorted the school bus to the Brown Middle School. At Brown Middle School, I also spoke Principle Kim Lysaght. The 48 students that were on the school bus were escorted to the auditorum of the school by staff on arrival. They were all accounted for and evaluated by the school nurse. No injuries were reported and the students were released to attend classes. Brown Middle School provided me with a roster of the students on the school bus at the time of the crash. I spoke with the operator of the school bus, Mr. Adriano Docanto (S08942704). Mr. Docanto stated he was operating a 2018 Thomas School Bus (MA BUN: BU43164) at the time of the crash. Mr. Docanto stated he was attemtping to make a right turn from the Boylston Street (E) offramp onto Parker Street, when the rear passenger side of his bus made contact with the front driver side of a pick up truck parked on the right side of the roadway. Mr. Docanto reported no injuries. I observed minor damage to the rear passegner side of the school bus. The school bus is owned by Eastern Bus Company and is contracted by the City of Newton. I spoke with the owner of the other vehicle involved, Mr. David Nowlan (S18764520). Mr. Nowlan stated he is a contactor for National Grid and was conducting a survey of the immediate area where the crash happened for a reported gas leak. Mr. Nowlan stated his 2017 Toyota Tacoma (MA: 7WDW0) was parked unoccupied on the Boylston Street (E) offramp before Parker Street at the time of the crash. Mr. Nowlan stated the rear passenger side of the bus sidewiped the front passenger side of his vehicle as it approached Parker Street. I observed damage to the front drive side door/fender/bumper area of MV2. Photos were taken of both vehicles and submitted to the IT Bureau.

HE STATED HE WAS TRAVELING SOUTH ON WALNUT ST AND SIDESWIPED MV1'S DRIVERS SIDE MIRROR AS HE WAS PASSING. NO INJURIES WERE REPORTED AND NO DAMAGE TO MV 2. IT SHOULD BE NOTED THAT THE ROADWAYS ARE MUCH NARROWER AFTER A SIGNIFICANT SNOWSTORM OCCURRED OVER THE WEEKEND. BOTH OPERATORS EXCHANGED INFORMATION AND WERE ADVISED I WOULD DOCUMENT THE INCIDENT WITH A POLICE REPORT.

On Wednesday, February 16, 2022, while assigned to Traffic unit N525, I responded to the intersection of Cabot Street and East Side Parkway for a report of a motor vehicle/bicycle crash. The weather at the time of the crash was clear and sunny. The road surface was dry. Cabot Street and East Side Parkway are both public ways maintained by the City of Newton. I spoke with the operator of MV1 (2016 Mazda CX3, NY: JNB6711), Mr. Tushar Singhal (NY: 287647610). Mr. Singhal stated he was operating his vehicle on Cabot Street (E) towards the East Side Parkway intersection. Mr. Singhal stated he came to a complete stop at the stop sign on Cabot Street. Mr. Singhal stated he did not see any vehicles or pedestrians in the area at this time. Mr. Singhal stated he proceeded to travel through the intersection from his stopped positon and take a left turn onto East Side Parkway. Mr. Singhal stated at this time a bicyclist crashed into the passenger side of his vehicle. Mr. Singhal stated the bicyclist then fell over his hood and onto the roadway. Mr. Singhal reported no injuries from the crash. I observed damage to the front passenger side door, mirror, fender, and hood. Sgt. M. Wade spoke with the injured bicyclist, Keiji Imai, while he was being evaluted by Newton Medics. Mr. Imai stated he was operating his Trek bicycle on Cabot Street (W) towards the East Side Parkway intersection. Mr. Imai stated he was on the sidewalk and entered the intersection with out stopping. Mr. Imai stated he crashed into the side of MV1 and fell to the roadway. Mr. Imai was transported to Newton Wellesley Hospital with minor injuries to his left wrist and rear. Keiji was wearing a helmet at the time of the crash. I observed the helmet to have no fresh damage or defects. Photos were taken of the helmet to document it's condition. Keiji's father, Koguke Imai, was made aware of the crash involving his son and stated he would respond to the hospital. I observed no substantial damage to the Trek bicycle. I transported Keiji's blackTrek bicycle to Keiji's residence. I spoke with a witness to the crash, Ms. Madison Albano. Ms Albano stated she was walking her dog on Cabot Street (E) on the Northbound sidewalk at the East Side Parkway intersection. Ms. Madison Albano stated she observed MV1 approach the intersection to her right side on Cabot Street (E) and come to a stop at the stop sign. Ms. Albano stated she then observed MV1 make a left turn onto East Side Parkway and she heard a crash. Ms. Albano stated she observed a bicyclist crash into the passenger side of MV1 and then fall to the roadway. Ms. Albano stated she did not see the bicyclist prior to the crash and does not know if he came off of the sidewalk or was in the roadway. After speaking with all involved parties, I have summized the following. As MV1 was completing his left turn onto East Side Parkway (N) from Cabot Street (E), the involved bicylist failed to yield at the stop sign while traveling on Cabot Street (W) across the intersection. The bicyclist entered the intersection with out yielding and crashed into the passenger side of MV1. MV1 had the right of way. No citations will be issued as a result of this crash. Photos were taken of the scene and submitted to the IT Bureau. Keiji is a student at Newton North High School.

On 03/08/22 at 14:57 hours, N494 and I Traffic Unit # 523 responded to the Horace Mann School located at 225 Nevada St. for a two car motor vehicle accident involving one of the operator's being struck. The accident occurred on Nevada St. just past a driveway of the Horace Mann School approaching the intersection of Nevada St and Linwood Ave. The weather at the time was sunny/clear and the road conditions were dry. School dismissal is at 14:50 hours and for approximately ten minutes Nevada Street is very congested with vehicles parked on both sides of the street. On arrival, I spoke to the operator of M/V # 2 identified as James Rice (MA D/L # S83251000). Mr. Rice's vehicle is a 2012 Ford Escape color white bearing MA reg. # 6725ZA. The Ford Escape was parked just past a school driveway facing southbound in a legal parking spot. He says he was picking up his child from the school. He further states that he looked and thought it was clear to open his door. He then opened his door and put his left foot and left arm out attempting to exit the vehicle when it was struck by M/V # 1. M/V # 1 struck the outer edge of the door and Mr. Rice's left elbow was also struck during the collision. Mr. Rice complained of left arm pain so Officer Boudreau had Fallon Ambulance respond to the scene. Rice then signed a patient refusal with them. Also inside the vehicle at the time of the crash was his son, Charger Rice. His son was seated and secured in a car seat in the second row of the driver's side of the Ford Escape. There was no injuries reported for him. I observed a crease to the outside edge of his driver's side door. Mr. Rice also stated to me that the crossing guard told him the other operator was speeding before the crash going approximately 25 miles per hour. Next, I spoke to the operator of M/V # 1 identified as Olga Kulakova (MA D/L# S63003743). She was operating a 2015 Infiniti QX60 color black bearing MA reg. # 5TJ151 at the time. Ms. Kulakova states that she was parked approximately twenty five feet back from the crash location also picking up her daughter from the school. After doing the pickup, she then proceeded out in to roadway going southbound on Nevada St. As she was approaching M/V # 2, she says the operator suddenly opened their door and she was not able to react in time striking his car door. She did not believe she ever struck the operator though. No injuries were reported. I observed white paint scrape marks to both her passenger side doors and the side view mirror was also damaged. I then spoke to the crossing guard assigned to the intersection of Nevada St. and Linwood Ave. The guard Jack Boudreau is employed through the City of Newton. I asked him what had occurred with the crash. He stated that he had his back turned away from the crash location when it occurred. He heard the collision which made him turn around. He then motioned for the driver of the Infiniti to pull over and stop. I asked him if he ever saw the Infiniti driving prior to the crash and he stated that he did not. I then inquired about the supposed speeding comment that he made to Mr. Rice regarding the Infiniti. He stated that he never said that to him. Digital photos were taken of the scene and submitted to the I.T. Bureau.

On Tuesday 3/15/22 at approximately 823 hours while assigned to marked unit n498 I was dispatched to the area of Countryside School (191 Dedham Street) for a report of a minor motor vehicle crash. Upon arrival I spoke with the operator, EBRIL-LEL Margarita, of motor vehicle #1, (695DE1). She states her vehicle was parked along the Northbound Side of Dedham Street diagonally across from Countryside School. She was at the back of her drivers side with the rear door open. She states while leaning into the vehicle her door was struck causing damage to the drivers side of her vehicle and rear drivers side door. After speaking with EBRIL-LEL I spoke with the operator, KARTASHOVA Natalia, of motor vehicle #2, ( 5LAA458). She states she was trying to drive passed MV#! that was parked on the side of the road and thought she had enough room between the oncoming traffic and the MV#1. She states EBRIL-LEL opened the door at the last minutes and she didn't have time to avoid hitting the door. During this time of the morning this area is extremely busy with both motor vehicle traffic and pedestrian traffic as it is drop-off for Countryside school. Cars are permitted to park on both side of the street in order to drop their children off at school. The parking situation causes narrow passage lanes

On Thursday April 7th, 2022 at approximately 1408 hours I responded to the area of 755 Dedham Street or a motor vehicle accident involving a school bus. At the time of the accident the weather was cloudy and the road surface was dry. Dedham Street is a public way owned and maintained by the city of Newton. Upon arrival I was informed that the school bus was not a city contracted bus and was from the town of Needham. There were no students on the bus at the time of the accident, just the bus driver identified as Charles Jerome and the bus monitor identified as Iola Karen Blake. Charles stated he was stopped in a line of traffic when the bus was struck from behind. No injuries were reported and there was minor damage to the rear end of the bus. The operator of the other vehicle involved was identified as Ariel Fengler. Ariel stated she was traveling behind the school bus and attempted to step on the brakes as the traffic was slowing down. Ariel stated she has a boot on her foot from a previous injury and the boot got stuck under the pedals and she panicked and could not stop her vehicle from hitting the school bus. Ariel stated she was not injured and her vehicle suffered heavy front end damage. Her vehicle was able to leave the scene and no tow was needed.

On April 9th 2022, at approximately 0038hrs, while assigned to cruiser N494, I, Officer Dragone, along with Officer Stake (N491) and Sgt. Brooks (N505) responded to the area of 196 Tremont St (public way in the City of Newton) for a report of a motor vehicle accident. Dispatch reported that the reporting party's vehicle was hit while it was parked in front of his address, and the operator of the other vehicle exchanged partial information and then left the scene. Upon arrival, I observed a blue Honda Accord bearing Massachusetts Registration: 1TDF63 (Vehicle 1) parked on the side of the road, blocking the driveway to 196 Tremont St. The vehicle had heavy damage along the driver's side of the vehicle and back left tire was pushed into the wheel well. In front of 190 Tremont St, I also observed a black BMW 528 bearing Massachusetts Registration: 1FDB38 ( Vehicle 2) with heavy damage along the passenger side of the vehicle and the front right wheel was completely detached from the axel. All the airbags in the vehicle were deployed, and it appeared that vehicle 2 was traveling Eastbound on Tremont St when it crashed into vehicle 1 at an angle. It should be noted that there were multiple deep scratches/dents on the hood, roof, and drivers side panels of vehicle 2 that were not consistent with the crash that just occurred; which indicates the operator was in multiple accidents prior to this one. Pro-tech Towing was also on scene to tow vehicle 2. I then spoke to the owner of vehicle 1, Ryan Desrosiers, who stated that his vehicle was parked on the side of the road when he heard a crash outside at approximately 2345hrs. Mr. Desrosiers stated when he went outside, he discovered his vehicle pushed in front of his driveway, approximately 10-15 feet from where he parked it, and the operator of vehicle 2 standing outside. Mr. Desrosiers stated when he spoke to the operator, he smelled like alcohol and had slurred speech. The operator, later identified as, Benjamin Wadsworth, told Mr. Desrosiers that he was "coming from in town" and "probably should have taken a Lyft tonight." When Mr. Desrosiers tried to exchange information, Mr. Wadsworth took Mr. Desrosiers' phone number and then texted him a link to a generic RMV website. Mr. Desrosiers then said Mr. Wadsworth called a tow company for his vehicle and insisted on paying out of pocket tomorrow for the damage to Mr. Desrosiers' vehicle but told him if he wanted he could call his insurance company. According to Mr. Desrosiers, Mr. Wadsworth then walked Eastbound on Tremont St towards Brighton. It should be noted that Mr. Wadsworth did not provide his name, address, or insurance company to Mr. Desrosiers prior to leaving. I had dispatch run a query on vehicle 2 (MA Reg: 1FDB38) and it came back registered to Benjamin Wadsworth out of 13 Charmane Rd East Falmouth, Massachusetts. While speaking with Mr. Desrosiers, the driver of the tow truck called Mr. Wadsworth cell phone to notify him that he was not going to be towing his vehicle. I was then able to get on the phone and speak with him, and he identified himself as Benjamin Wadsworth and admitted to being involved in the accident that just occurred. When I asked Mr. Wadsworth his location because I was concerned for his well-being due to the severity of the crash and airbag deployment, he stated he was at his apartment in Brighton. When I asked for the address of his apartment, he refused to give it to me and stated he was fine. I stated to Mr. Wadsworth that because he only gave limited information and then left the area that he could be charged with Leaving the scene of a collision, to which he replied, "I am in law school you can see the books in the back of my car, you can't do that, you have my plate, he can look that information online." When asked about having an address out of Falmouth, Mr. Wadsworth stated he has two addresses because he attends school in the area. It should be noted that while speaking with Mr. Wadsworth on the phone, his speech was slurred, slow, and dilerbrate, consistent with being intoxicated. Tody's towing arrived on scene and took possession of Mr. Wadsworth's vehicle. I then tried a second time to speak with Mr. Wadsworth via a phone call to get him to agree to a well-being check. Mr. Wadsworth, again, refused to provide me with his address in Brighton, insisting he was fine. Although Mr. Wadsworth sounded intoxicated, he did not sound injured and was coherent when answering my questions. Due to Benjamin Wadsworth operating recklessly, knowingly getting into a collision causing property damage, not providing his name, not providing his address, and leaving the scene, I am issuing him Massachusetts Uniformed Citation T1445620 for the following: MGL. Ch. 90 S.24 (Leaving the scene of a collision after property damage) MGL. Ch. 90 S.24/E (Negligent operation of a motor vehicle) A Request for Immediate Threat License Suspension/Revocation form was filled out and faxed over to the RMV.

Operator of vehicle#1 stated that as she was travelling east bound on Pearl St closest to the intersection of Jackson Rd, she observed vehicle #2 drive Southbound on Jackson Rd, go though the intersection red light without stopping and proceed driving right in front of vehicle #1's path of travel. Operator of vehicle#1 stated that vehicle #2 actions made it impossible for her to avoid contact with vehicle #2. Operator of vehicle#1 stated that after the accident took place, Vehicle#2 then proceeded to drive away east bound on Jackson Rd without stopping. Operator of Vehicle #1 stated that she activated her cruiser blue lights and proceeded to pull into the Jackson school driveway (#150 Jackson Rd) Operator of vehicle #1 stated that she then approached the Operator of Vehicle#2 and told her that she can't leave an accident scene without trading information, to which the operator of vehicle#2 stated to her, "sorry, I'm late to pick up my kid." Operator of vehicle#1 stated that the operator of vehicle#2 then went inside the school and then retrieved her child and returned. Operator of vehicle#1 had no visible injuries, but stated that she believed she may of hit her head on the steering wheel due to the accident. Operator of vehicle#1 was transported to Newton Wellesley Hospital by paramedics for further medical evaluation. Operator of Vehicle#2 at first stated she did not remember what happened during the accident. Operator of vehicle#2 then stated that she was stopped at the red light on Jackson Rd at Pearl St and when the light turned green she proceeded through the intersection and vehicle#1 ran the red light and crashed into the side of her vehicle. Operator of vehicle#2 stated that she may have some leg pain from the accident but did not want to go to the hospital. I asked Operator of vehicle #2 after the accident if she stopped to talk to the other operator and she admitted that she did not and proceeded to go to the Jackson School and retrieve her child. Operator of vehicle#2 stated that she was in shock after the accident happened and wasn't thinking straight, and she also stated that this was her first motor vehicle crash and was worried about being late picking up her child. I observed damage to passenger side doors of vehicle #2 and the operator of the vehicle was going to wait for a private tow. I issued a MA uniform citation (#T2017025) to the Operator of Vehicle#2 for a criminal application of ch 90 s 24 Leaving the Scene property damage. Traffic Officer Iarossi (535) arrived on scene and took pictures of both involved vehicles and the intersection. Traffic Officer Iarossi and I canvassed the surrounding houses and buildings for any witnesses or video and had negative results.

AS HE WAS APPROACHING THE INTERSECTION OF WALNUT STREET, MV2 ATTEMPTED TO CHANGE LANES (GOING FROM THE LEFT LANE INTO THE RIGHT LANE) AND TURNED INTO MV 1, STRIKING HIS VEHICLE ON THE FRONT DRIVERS SIDE. MODERATE DAMAGE SUSTAINED TO MV 1 AND NO INJURIES WERE REPORTED. OPERATOR OF MV 2 STATED HE WAS ON WASHINGTON ST HEADING EAST ON WASHINGTON STREET IN THE LEFT LANE AT THE INTERSECTION OF WALNUT ST. HE STATED THAT HE LOOKED TO HIS RIGHT AND DID NOT SEE ANY VEHICLES TRAVELING IN THE RIGHT LANE SO HE ATTEMPTED TO MAKE A LANE CHANGE. WHEN HE ATTEMPTED TO TURN INTO THE RIGHT LANE, HE STRUCK MV 1 ON THE FRONT DRIVER SIDE. MODERATE DAMAGE SUSTAINED AND NO INJURIES WERE REPORTED. THREE STUDENTS ON THE BUS WERE CHECKED OUT BY MEDICS (REFUSALS), PARENTS WERE NOTIFIED AND THEY WERE SENT ON THEIR WAY TO SCHOOL (CLEARWATER SCHOOL ON WASHINGTON ST).

On Monday, May 9, 2022, while assigned to Traffic unit N525, I responded to the area of 25 Berkshire Road, Newton, for a report of an MVA involving a City of Newton owned street sweeper and a City of Newton contracted school bus. The weather at the time of the crash was clear and sunny. The road surface was dry. Berkshire Road is a public way maintained by the City of Newton. The operator of MV1, Mr. Norman Fryar (S20854611), stated he was operating a City of Newton owned 2017 Elgn Pelican street sweeper (MA: MVN M96170) on Berkshire Road. Mr. Fryar stated as he was sweeping the street he was in the Eastbound lane traveling North. Mr. Fryar stated there was a vehicle parked ahead of him and to avoid it he began to change lanes to his left to go around it. Mr. Fryar stated as he was chaning lanes his phone was ringing and as he reached for the phone the driver side of his sweeper crashed into the front passenger side of a passing school bus. Mr. Fryar reported no injuries. I observed damage to the driver side of the sweeper. I spoke with the operator of MV2, Ms. Ana Mendes (S72311538). Ms. Mendes stated she was operating a City of Newton contracted school bus (MA: BU44536) on Berkshire Road with 25 students on board with a final destination of Bigelow Middle School. Ms. Mendes stated she was traveling North on Berkshire Road when she observed a street sweeper in front of her. Ms. Mendes stated she entered the Westbound land of Berkshire Road to go around the street sweeper. Ms. Mendes stated as she was passing the street sweeper, it made an abrupt left turn and it's driver side crashed into the front passenger side door/mirror of the school bus. Ms. Mendes reported no injuries. I observed moderate damage to the passegner side of MV2. After the initial crash, it was reported by both parties that MV1 attempted to back up to allow students to exit the bus through the passenger side door. As MV1 was backing up, it crashed into the middle passenger side of the school bus. There were 25 Bigelow Middle School students on the bus at the time of the crash. Ms. Mendes state she contacted her dispatch for further instructions on what to do with the students. Ms. Mendes stated to Sgt. Wade that the Eastern Bus Company dispatch instructed her to keep the students on the bus and wait for Police and EMS to arrive. Ms. Mendes stated after speaking with her dispatch, another school bus arrived at the scene. The decision was made by Ms. Mendes to have the students exit her school bus and be transported to Bigelow Middle School prior to my arrival. Ms. Mendes was advised in the future to follow the policy set forth by her Eastern Bus Company, the City of Newton, and the Newton Police Department. After clearing Berkshire Road, myself and Sgt. M. Wade responded to the Bigelow Middle School to check on the welfare of the students that were on the bus at the time of the crash. We were greeted by the school's Asssitant Principle Mr. Lucas Mogensen. Mr. Mogensen stated all of the students that were on the bus at the time of the crash were staged on the bus when it arrived at the school. Mr. Mogensen stated the school nurse Ms. Shannon Loughnane evaluated each student as they exited the bus and there were no injuries reported at this time.

On Monday May 16, 2022 at 0848 hours vehicle 1 was stopped in front of #188 Lexington St waiting to pick up a child. While parked vehicle 1s drivers side mirror was hit by a passing vehicle traveling southbound towards Commonwealth Ave with out stopping. The operator of vehicle 1, Claire Rice was driving for JSC Transportation and had one child aboard, Amir Ayden-Cronin who was being transported to Newton Early Childhood on Jackson Rd. Neither Rice or Ayden-Cronin reported any injuries and signed patient refusals with Fallon medics. Sahar Zaheer (617-943-8356) is the mother of Amir and I spoke to her over the phone. Zaheer was notified of the situation and gave consent for medics to evaluate her son and for him to continue to school for the day. I contacted Susan Downs (781-272-8555) of JSC to attempt to see if the vans dash camera captured any of the incident but have not heard back at this time. Rice had already spoken to her dispatch who did not believe the camera would have been clear enough to pick up anything to assist in identifying the other operator. Officer Gaudet took pictures of the damage to the vans mirror.

On Wednesday, June 1, 2022, while assigned to Traffic unit N525, I responded to the intersection of Commonwealth Avenue and Temple Street, Newton, for a report of a motor vehicle crash involving a City of Newton contracted school bus with students onboard. At the time of the crash there was a light rain falling with overcast conditions. The road surface was wet. Commonwealth Avenue and Temple Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Ms. Brenda Keefe (S95491012). Ms. Keefe stated he was operating a 2018 Ford Transit van (MA SPN: SP109407) owned by JSC Transportation at the time of the crash. JSC Transportation is contracted by the City of Newton to transport Newton Public School students. Ms. Keefe stated she was traveling on Commonwealth Avenue (W) past Temple Street when a vehicle exited Temple Street right in front of her vehicle. Ms. Keefe stated she did not have time to stop and avoid the collision. The front end of MV1 crashed into the driver side of MV2. At the time of the crash, there were two students from Zervas Elementary School being transported by Ms. Keefe and a bus monitor, Ms. Emily Scofield (S96767671). The students being transported were Andre Toossi Zacarias and Daniel Alvarado. No injuries were reported by any of the occupants of MV1. Daniel and Andre were evaluated by Newton Medics and transported from the area by a parent. I observed significant damage to the entire front end of MV1. Tody's Towing responded and removed the vehicle from the roadway. The operator of MV2, Mr. William Harrigan (S77851597), stated he was operating his 2009 Toyota Yaris (MA: 2YGG77) on Temple Street (N) towards Commonwealth Avenue. Mr. Harrigan stated he came to a stop at the intersection on Temple Street, and then proceeded to enter Commonwealth Avenue to take a left turn. Mr. Hannigan stated when he entered Commonwealth Avenue and did not see MV1 traveling towards him. Mr. Hannigan stated MV1 crashed into his driver side. I observed significant damage to the entire driver side of MV2. Mr. Harrigan initially signed a patient refusal with Newton Medics, but then was transported to Newton Wellesley Hospital for evaluation. Tody's towing responded and removed MV2 from the roadway. The operator of MV3, Ms. Mary Potere (S96201952), stated she was traveling on Commonwealth Avenue (E) towards Temple Street in her 2018 Toyota Camry (MA: 18FW37). Ms. Potere stated MV2 entered the roadway in front of MV1 as it was traveling on Commonwealth Avenue (W) from Temple Street (S), and MV1 could not avoid the collision. MV2 crashed into MV3's driver side after MV2 was hit by MV1. Ms. Potere was transported to Newton Wellesley Hospital by Newton Medics for a potential head and neck injury. I observed damage to the front end and driver side of MV3. Tody's towing responded and removed MV3 from the roadway. I spoke with a witness to the crash, Ms. Susan Flicop. Ms. Flicop stated she was traveling behind MV1 on Commonwealth Avenue (W) at the time of the crash. Ms. Flicop stated she observed MV2 stopped on Temple Street ahead of MV1, and MV2 was "edging out" onto Commonwealth Avenue. Ms. Flicop stated MV2 entered Commonwealth Avenue in front of MV1, and MV1 did not have time to stop. Ms. Flicop stated MV1 crashed into MV2's driver side and pushed the vehicle back. Ms. Flicop stated at this time, MV3 was traveling on Commonwealth Avenue (E) and was hit by one of the vehicles. Ms. Flicop stated once MV2 came to a stop in the roadway, it rolled back onto the medium off the roadway. After speaking with all parties involved, I have surmised that Mr. Harrigan failed to use care while entering the roadway prior to the crash. Mr. Harrigan did not yield to MV1's right of way while she was traveling on Commonwealth Avenue, resulting in the collision. Mr. Harrigan was provided with Massachusetts Uniform Citation 455661AB in hand for Chapter 89, Section 8 (Fail to Yield to Right of Way), and Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care). Photos were taken of the crash scene and submitted to the IT Bureau.

On 06/06/22 at 15:12 hours, I was at the Cabot School on Bridges Avenue doing a school crossing guard post when I was flagged down by a witness about a hit and run motor vehicle crash that occurred a few minutes prior. The crash took place in the school driveway/parking lot that runs between Bridges Avenue and Norwood Avenue. The driveway/parking lot is a one way roadway that enters through Bridges Avenue and exits out to Norwood Avenue. It allows staff to park their vehicles there and it is used for school pickup and drop off. The witness identified as Michelle Reddick stated that she was in her vehicle in the school pickup line to the above mentioned driveway of the school. Ms. Reddick observed a black colored Mercedes GLE450 bearing MA reg.# 2AWR85 come in through the one way driveway going the wrong way. The Mercedes then attempted to back up in to a parking spot reportedly three times with two other vehicles parked on each side. While doing so, the operator struck the rear end of M/V # 2. The operator of M/V # 1 was described as an Asian female. Ms. Reddick stated that other parents saw the accident but nothing was said to the operator. Reddick beeped her horn at the operator to get her attention but the Mercedes pulled away and left the scene without leaving their information. Reddick took a picture of the vehicle (Mercedes GLE450) and registration (MA 2AWR85) and showed it to me. M/V # 2 was a 2020 Nissan Rogue color gray bearing MA reg. # 598VN7. The owner, Elizabeth McFadyen, was made aware of the crash and provided the owner's information from the Mercedes. The vehicle sustained minor scratches to the left side of the rear bumper. The left side rear quarter panel/bumper area was no longer mounted flush with the rest of the body also. The owner of the Mercedes is an Asian female listed as Qiong Yang out of 480 Main Street Apt. # 113 Malden, MA. I was not able to locate a phone number for her and Malden Police Department had no contact info either. A department letterhead was sent to the address requesting the owner to contact me regarding the crash.

Narrative:
On 06/15/22 at 14:47 hours, I (Traffic Unit # 523) responded to Cragmore Road (public way) and Boylston Street for a hit & run motor vehicle crash involving a yellow school bus and a tree company truck. The weather at the time was cloudy and sunny and the road conditions were dry. It appears the crash occurred at approximately 14:30 hours on this date according to statements. On arrival, I spoke to an employee identified as Alan Hayden of the tree company. Mr. Hayden works for Tree Technology and Landscape Company Inc. and at the time of the crash he was up in the air inside the bucket of his truck conducting tree trimming work. No other workers were inside the truck at the time. The truck is a 2022 Freightliner M2 bearing MA commercial reg. # W23962. Hayden states the truck was parked at the end of Cragmore Road a bit out into the Boylston Street off ramp. While doing work, Hayden states a full size yellow school bus attempted to go around his truck while turning right on to the Boylston Street off ramp from Cragmore Road. The school bus did not make a wide enough right turn and struck the driver's side black metal cage which supports the bucket to the truck. The bucket then started shaking back and forth while Mr. Hayden was in it. The metal cage goes overhead in front of the truck and sticks out on both sides of it. He says that the school bus scraped along the side of the cage and there should be long sized scrape marks along the top right side of the bus. I observed the scrape marks to the cage and noticed that the cage was also bent outward slightly on the passenger side. No injuries were reported by Mr. Hayden. I took several photos of the Freightliner truck and cage area and submitted them to the I.T. Bureau. Two other employees for Tree Technology were outside of the truck observing the work being done at the time. They were identified as Edgar Ortiz and Kemwrick Thames. Both of them did not observe the collision but heard the school bus scrape along side their company's truck. They stated that they yelled out to the bus operator but the driver continued westbound on to Boylston Street without stopping. Neither of them saw any writing on the bus or a registration number for it. They did observe some type of paper sign attached to the side of it. They stated that numerous students were on the school bus at the time. They did not observe the sex of the bus operator. Dispatch attempted to contact the City of Newton Transportation Dept. with negative results. Sgt. Wade left a voicemail with the director Regina Moody to see if the bus involved was transporting students for the City of Newton/Public Schools. It is unknown at this time if this bus was contracted by the city. Traffic Bureau update (Officer Gaudet): On Thursday, June 16, 2022, I was asked to conduct a follow up investigation for this crash. I spoke with Ms. Regina Moody from the City of Newton Transportation Department. Ms. Moody identified the operator of the school bus as Mr. Urban Mills (S49560217) and stated he was operating bus #351, a 2019 Thomas School Bus (MA BUN: BU43104). Ms. Moody was able to provide me with a copy of the audio/video surveillance from the bus involved in the crash. I viewed this footage in Ms. Moody's office. The footage shows Mr. Mills operating the school bus on Cragmore Road (S) towards Boylston Street. As the bus approached Boylston Street, a tree trimming crew can be seen working ahead at the corner of Cragmore Road and Boylston Street. There is also a stop sign at this intersection that can be clearly viewed from the footage. As the school bus approaches the intersection it does not come to a stop at the stop sign posted on the right side of the roadway. This was confirmed by Ms. Moody using the GPS system that gives the speed of every bus traveling during it's route, including when it comes to a complete stop. The school bus takes a right turn and a loud "crunching/scraping" sound can be heard. The attention of the children on the bus is turned to the passenger side of the bus. Through my training and experience, the sound heard on the footage is consistent with a vehicle hitting a large object. Children on the bus can be heard yelling, "What the fuck!" and discussing what has just happened with one another. The bus completes it's turn and travels on it's way with out stopping. All of the students on the bus were dropped off at their usual destination. There have been no injuries reported by any of the parties on the bus at the time of the crash. All of the students on the bus at the time of the crash were from Brown Middle School. Ms. Moody stated she would contact the Brown Middle School to try and get a list of the students that were on the bus at the time of the crash. A copy of the footage from the school bus and pictures of the damage to bus #351 was submitted to the IT Bureau. Mr. Mills provided a statement to Ms. Moody sometime after the crash occurred. From Mr. Mills's written statement, he states, "On approaching the intersection, I noticed a couple of tree cutting vehicles parked on the right side of the roadway (Cragmore Road) at the very corner. Nevertheless, I negotiated the turn and proceeded. This will require me to maneuver my bus onto the sidewalk. While executing this maneuver, I heard some noises which at the time I thought it was as a result of the bus getting onto the sidewalk because while executing the maneuver my eyes were fixed on the right side mirrors and was a great distance away from the vehicles to my right." After viewing the footage from the crash, Mr. Mills will be mailed Massachusetts Uniform Citation 477181AB for Chapter 90, Section 24 (Leaving the Scene of Property Damage), Chapter 89, Section 9, (Fail To Yield, Stop Sign), and Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). A copy of the criminal application and associated paperwork was submitted to the Police Prosecutor. Ms. Moody was apprised of the findings of my investigation. Cragmore Road and Boylston Street are both public ways in the City of Newton.
