On 01/15/2019, at 1919 hrs, I responded to the intersection of Washington St and Centre St for a two car MVA. Upon arrival, I observed heavy damage to the front end of MV1 and moderate damage to the rear end of MV2. The operator of MV2 complained of head/neck injuries and was transported to Newton Wellesley Hospital by Cataldo for an evaluation. The operator of MV1 stated she was attempting to merge onto Washington St from Centre St when she rear ended MV2. The operator of MV2 stated when she slowed to merge onto Washington St from Centre St she was rear ended by MV1. Both vehicles were towed by Tody's. Towed MV inventory forms were filled out and filed.

COLGATE stated although MONIR did give him his lic and reg information he left prior to my arrival.

MV1 operator states she was driving Eastbound on Washington St when MV2 attempted to cross intersection, resulting in collision. MV2 operator states she had pulled up to the stop sign from the I-95 off ramp and was allowed to enter into the first lane of traffic. She then states the operator of MV1 slowed her vehicle down and then began to cross the second lane of traffic. This resulted in a collision of both vehicles. No injuries were reported on scene. Both vehicles were able to be driven away. I then cleared without further incident.

At approximately 1630 HRs on Wednesday, January 23, 2019, I was dispatched to the intersection of Washington St. and Stanton Ave. Upon arrival, both parties had pulled into 160 Stanton Ave. and began to exchange paperwork. I spoke with the operator of MV1 (MA Pass: 166AW2), who stated that as she traveled Eastbound on Washington St. she was cut off by MV2, who was attempting to cross over Washington St. and enter Stanton Ave., causing her vehicle to collide with MV2. I spoke with the operator of MV2 (MA Pass: 822EZ4) who stated that she was stopped at the intersection and made a left hand turn in an attempt to enter Stanton Ave Southbound, believing she had enough time to safely do so, it was at this point that her vehicle and MV1 collided. I observed damage to MV1's front right bumper area and damage to MV2's right rear door area. Both parties stated that they were uninjured and I cleared without incident.

Operator of vehicle 1 stated he was traveling eastbound on Washington st and as he approached the intersection of Prospect St could not recall if the light was red or green but still preceded forward and was struck by vehicle 2. Operator of vehicle 2 states he was sitting on Prospect St at the light on Washington St. When the light turned green he proceeded forward across Washington St when vehicle 1 came across his path and he struck it on the driver side. Vehicle 1 had side airbag deployment. Both vehicles were towed by Todys. Both operators were evaluated by the medics and signed patient refusals.

On 12/9/2019, at 1526 hours, I responded to the intersection of Washington St and Lewis Ter for a two car MVA. Upon arrival, I observed damage to both vehicles front ends. No injuries were reported. The operator of MV1 stated she was on Washington St (WB) attempting to take a left onto Lewis Ter when she collided with MV2 who was traveling EB on Washington St. The operator of MV2 stated he was traveling eastbound on Washington St when MV1 turned in front of him and they collided. It appears that MV2 had the right of way. MV1 was towed from the scene.

On February 28th, 2020 at approximately 11:27 hours while assigned to marked cruiser N491 I responded to #433 Centre St for a two car MV crash. I later located both operators in the rear parking lot to Marathon Physical Therapy. I first spoke with operator #1, who was driving a Honda Pilot. She reported she was driving E/B on Washington St and was veering right to go over the Centre Ave bridge when she reports vehicle #2 cut into her lane causing the crash. She further reported that she was slowing as she entered onto the bridge looking for merging traffic coming from W/B traffic on Washington St when vehicle #2 came from her rear, to her drivers side and cut into her lane. I then spoke to operator #2 who was operating a small Subaru Impreza. He reported that he was first driving E/B on Washington St and was also veering right onto the Centre Ave Bridge and had slowed/stopped on the bridge as he was looking for the merging traffic coming from the W/B side of Washington St. He then reports that vehicle #1 came from his passenger side and cut into his lane causing the crash. Since both vehicles had already moved from the crash location prior to my arrival and both operators had different versions of how the crash occurred, I couldn't determine who was responsible for this crash. I left both vehicles separated in the crash diagram since I was never told which lane they were in prior to entering onto the bridge from Washington St plus their statements were completely different as to who was responsible. There were no injuries and neither vehicle required a tow.

On 6/26/2020 at approx 1214hrs while assigned to 497 I responded to the area of Washington St at Quinobequin Rd for a report of a 2 car crash with air bag deployment. Upon arrival I observed Ma Reg 579AM3 a gray Toyt Rav 4 and Ma Reg 219w70 a red Toyota Corolla in the middle of the EB lanes of Washington St both with significant damage, front and side airbag deployment and fluids leaking. I ascertained the operators were not hurt and called for two tows and NFD for speedy dry. I spoke with the operator of the red Toyt Brendan OCONNOR who stated he was travelling EB on Washington St, had the green light, was proceeding through the intersection when he was struck by the operator of the Toyt Rav 4,Gabriella NUGENT. I spoke with NUGENT who stated she was attempting to take the left onto Quinobequin Rd from the WB lane of Washington St when she collided with OCONNOR. NUGENT acknowledged that although the light was green WB she did not have the left arrow to turn. Both cars towed by Todys, both Nugent and OCONNOR signed patient refusals with Cataldo ambulance.

The operator of vehicle 1 states she was traveling north on Chestnut Street, taking a left onto Washington Street. Operator of vehicle 1 states she has a green light, as she proceeded into the intersection vehicle 1 was struck on the left side by vehicle 2. The operator of vehicle 2 also claims he had a green light, while traveling east on Washington Street. The operator of vehicle 2 states as he proceeded into the intersection, so did vehicle 1. At that time vehicle 2 struck vehicle 1 with the front of vehicle 2. Medics responded due to airbag deployment and transported the operator of vehicle 2 to NWH for an evaluation. It should be noted at the time of the accident the glare while traveling east on Washington Street makes is difficult to see. Todys towed both vehicles.

On September 19th, 2020 at approximately 13:20 hours while assigned to N491 I responded to a report of a two car MV crash at the intersection of Washington St(E/B) at the Centre St bridge(N/B). On my arrival I spoke with operator #1, Charlie Margosian. He reported first going E/B on Washington St in the lane for traffic to veer left towards the Centre St bridge when MV #2 entered his lane from his right( passenger side) striking his vehicle from his front to his rear on the passenger side. Operator #2identified as Won K.Yun was heading E/B on Washington St in a lane that was for straight ahead traffic when he signaled to make a lane change to go towards the Centre St bridge when he collided with vehicle #1. He never saw vehicle #1 prior to changing lanes.

MV 1 was traveling straight Eastbound through a green light at the intersection of Washington St and Park St. At this time MV 2 entered the intersection and struck the front end of MV 1 decapitating the bumper. MV 1 stopped at the point of impact. The operator of MV 1 stated MV 2 continued traveling Northbound towards the direction of Watertown. MV 2's vehicle description was a white work van with some lettering on the side. The operator of MV 1 was unable to provide any plate information or what lettering was on the side of the van. The bumper was stored in the rear of MV 1 and the vehicle was driven away by the operator. MV 2 was traveling North on Park St and entered the intersection of Park St and Washington St. At this time MV 2 struck the front end of MV 1 and continued driving East towards Watertown.

The operator of MV1, Baruch Ticho, stated that he was traveling Northbound on Waverly Avenue and came to a complete stop at the stop sign before attempting to take a left turn onto Washington Street. Mr. Ticho stated that he did not see the bicyclist at anytime because she was wearing all dark clothes and collided with her while attempting to turn. The bicyclist, Ellen Messing, stated that she was traveling East on Washington Street in the marked bike lane and believed MV1 saw her and was letting her pass. The MV continued on after stopping at the stop sign and t boned her bicycle. Ms. Messing had no visible injuries but stated she had leg pain and was transported to the BI. Ms. Messing's bicycle sustained major damage so I transported it to the station and secured it in support services to be picked up by Ms. Messing. MV1 sustained minor driver side and front bumper damage and was driven from the scene. Officer Stake took photos of the scene which were submitted to IT to be attached to the report.

On 2/8/21 I was working N491 when I responded to an MVA PED on Washington St. at Crafts St.. Upon arrival I spoke with Dennis Delicata who stated that he was traveling southbound on Crafts St. when he turned left onto Washington St. during a green light a pedestrian later identified as Jamie Correia ran out in front of his truck and he struck him. Delicata stated the Jamie was traveling in the travel lane of Washington St. going Eastbound across the intersection not in the crosswalk and he did not see him in time. Delicata stated he slammed on his brakes however could not stop in time. Delicata stated he got out of his truck and rendered aid to Correia who was complaining of pain in his back and shoulder. Delicata stated that Correia said to him "I thought I could make it". I spoke with Jamie Correia who was being evaluated by the medics. Correia said he was running across the intersection headed Eastbound and thought he could make it across. Medics transported Correia to NWH. Delicata's vehicle suffered minor damage and was driven away from the scene. Pictures were taken and submitted to IT.

MV2 operator stated that she and the operator of MV1 were both in a road rage incident, operating eastbound on Washington St prior to Centre St at approximately 1910 hours. MV2 operator stated before she attempted to merge left, MV1 purposefully cut her off by merging right in front of her, and slammed on the breaks. Because MV1 slammed on the breaks by merging right, MV2 operator stated she struck the rear of MV1. MV2 operator reported no injuries at this time, and MV2 sustained minor front driver side hood and front driver side headlight damage. MV2 operator stated she believed MV1 may have sustained damage. MV1 operator stated he did not think he was involved in a motor vehicle accident. However, MV1 operator reported a road rage incident at that time and area, and stated he did recall MV2 taking up to travel lanes. MV1 reported no injuries at this time and upon looking at his vehicle, only reported very minor paint transfer. Please see incident report #21014753.

Upon arrival I met with Detail Sgt Selig who was working the Washington St detail. Washington St was being prepped for asphalting by D&R contracting and the double lanes were narrowed to single on Washington St from the east side of Commonwealth Av . When the operator of Ma Reg 41H170, Jessica Martin was trying to merge into the single lane her front right tire touched the rear of D& R Contractors low bed trailer causing her tire to go flat and very minor scrapping on her right front tire well.

On 8/12/2021at approx 0903 hrs while assigned to 497 I responded to the area of Washington St and Belmore Pk for a report of a two car crash with airbag deployment. Upon arrival i observed ma reg 2PSJ14 a white Honda Odessey and Ma Reg 2NZS96 a white Mazda CX3 in the median of Washington St with both operators out of their cars. I spoke with the operator of the Mazda, Milli Langston who stated she was attempting to take a left onto Washington St from the 95NB Off ramp, traffic in the southern eastbound lane of Washington St stopped for her, when attempting to cross she was struck by Istamar Levin who was travelling in the northern eastbound lane of Washington St with his two sons Asher and Zev Levin . All parties signed patient refusals ad were able to drive away.

On September 20th, 2021 at approximately 12:22 hours while working N491 I responded with Fallon Ambulance and NFD to a report of a MV crash on Washington St @ Park St with injuries. On my arrival I located both involved vehicles on Washington St near Park St. Vehicle #1 was a 2015 Grey Toyota RAV4, MA REG. 5AVE80, operated by a Marineide Lopes. She stated she was going E/B on Washington St @ Park St in the far left lane when vehicle #2 who was on her right, next lane over attempted to turn left in front of her causing the crash. The impact forced her vehicle to the left onto an island causing further damage to her vehicle. Vehicle #2 was a 2016 White Chev.Cruz MA REG. 8NWJ30, operated by a Michael Faraji. HE stated he was going E/B on Washington St when his GPS indicated to make a left turn (north)towards the Centre St Bridge. He then attempted to make the left turn striking vehicle #1 as it was passing the area that was open for only N/B traffic coming from Park St. There is no legal left turn for E/B traffic on Washington St Both vehicles were towed from the scene by todys. On 10/6/2021 at approx 0759Hrs while assigned to 497 I responded to the area of Washington St and 128N Off Ramp for a report of a two car crash without injuries. Upon arrival I observed Ma Con Reg R14581 a large tree removal truck fully in the southern eastbound lane of Washington St. Ma Reg 6PX766, a white VW Jetta straddling the fog lane and partially into the southern Eastbound lane.I spoke with the operator of the Jetta, Jordan Glickman who stated he was stopped in traffic when the tree truck struck him. I spoke with the operator of the tree truck, Leobardo Carrillo Reyes, who stated he was travelling east on Washington St when the Jetta came up the off ramp, to avoid eastbound traffic backed up on the exit he came up the left turn lane from the exit, took and illegal right turn onto Washington St and tried to cut into the trucks lane causing the collision. Leobardo also gave me the card of a witness , Michael Minkoff who had stopped and corroborated this version of events.

MV2 operator stated he was stopped on Washington St (eastbound) at Hope St intersection (both public ways). MV2 operator stated he stopped right before the intersection because there is road marking and posted sign that state "Do Not Block Intersection", and there was heavy traffic. MV2 operator stated as he was stopped in traffic his rear end was struck from behind by MV1. MV2 sustained superficial scrapes to the rear end. MV2 operator and passenger did not report any injuries at this time, no tow required. MV1 operator stated he was stopped behind MV2 on Washington St in heavy traffic. MV1 operator stated he then noticed MV2 moving forward in traffic. MV1 operator stated he pulled forward too but looked down at his radio to change the station. MV1 operator stated he looked up and immediately noticed MV2 stopped, but he was unable to stop in time. MV1 struck the rear end of MV2. MV1 sustained extensive and total front end damage. MV1 was towed by Todys. MV1 operator and passenger reported no injuries at this time. MV1 operator's parent came to the area and picked him and passenger up.

Operator of MV1 stated she was in the right lane on Washington St going around the rotary heading towards Centre St when the vehicle to her left tried to merge in her lane and struck the left side of her vehicle. The collision forced MV1 to strike the curb of the island on her right side and also damage the rims on the passenger side. Operator of MV1 then stopped at the red light and got out of the vehicle and the operator of MV2 told her she would pull over. Operator of MV2 began driving on Centre St and operator of MV1 believed she was not going to pull over so she called the police and left the scene. Operator of MV2 was able to meet myself and operator of MV1 at Dunking Donuts located at 343 Washington St. Operator of MV2 stated she was in the left lane when MV1 tried to merge into her lane. MV1 struck her vehicle and operator of MV2 believed MV1 was going to follow her to a safe location to exchange paper work. Operator of MV2 states she pulled off onto a side street and MV1 continued to drive straight. Operator of MV2 was not going to call the police and was going to just contact her insurance company. No parties needed medical attention. Neither vehicle required a tow.

The operator of MV1 stated he was traveling on Centre Street, North bound, when he stopped while entering Washington Street due to on coming traffic. When he stopped at the intersection of Centre Street and Washington Street, MV2 struck the rear of his vehicle. The operator of MV2 stated she was traveling North bound on Centre Street, approaching Washington Street. The light was green and MV1 began to yield on to Washington Street when MV1 stopped. The operator of MV2 then struck the rear of MV1 with her front driver side bumper. When I approached MV1, I did not observe any damage to the MV. I spoke with the driver and passenger. Both parties stated they were injured. When I asked what was injured they stated "everything". I then asked for the drivers license and registration. He handed me his license without any sign of pain. I requested two ambulances for the driver and passenger. When I returned to give the operator his license back, he stated he was unable to raise his arm now. The passenger was able to walk out into the ambulance. The driver needed assistance by the medics. Due to both parties requesting to be transported, Tody's responded. The operator of the towed vehicle would not provide Tody's with the key to the vehicle. Dave, was still able to tow the vehicle and he secured the doors prior to transporting the vehicle. Inside the vehicle their was numerous blankets, clothing and DVD's. A black cell phone was left in the Centre console. The operator of MV2 was not injured and I observed very minor damage to the driver side bumper. I checked MV1 again for any damage to the rear bumper and did not observe any damage.

On March 28th, 2022 at approximately 09:13 hours while assigned to N491 I responded to the intersection of Washington St @ Jackson Rd for a reported MV crash. On my arrival I located both involved vehicle E/B on Washington St @ Jackson Rd. Vehicle #1 was a 2019 Black Tesla X SUV, Ma reg. EV502N, operated by Jeffrey Cook. He stated he had just stopped on Washington St E/B due to the vehicle in front of him stopping by Jackson Rd. His vehicle was then rear ended by vehicle #2. Vehicle #2 was a 2020 Black Chev. Silverado PU, Ma reg. 2NBN74, operated by David Haber. HE stated he was behind vehicle #1 on Washington St E/B when vehicle #1 suddenly stopped in front of him causing him to crash into the rear of the vehicle. Vehicle #1 sustained heavy rear end damage. Vehicle #2 sustained heavy front end damage. Neither vehicle required a tow and no reported injuries at this time.

Operator of Vehicle #1 was stopped in heavy traffic on Cherry Street near the intersection with Washington Street. NPD Cruiser 502 was responding to an emergency with lights and sirens activated. Vehicle #1 was moving towards the shoulder of the road to yield to the emergency vehicle. Cruiser 502 struck the driver side rear panel and bumper. Operator #1 was not injured. On 06/17/2022, while assigned to N492, I, Officer Conary, responded to the area of 425 Centre street for a three car MVA. Upon arrival, all three cars were pulled over on the left side of the road on Washington Street. I spoke with Operator of MV1 who stated that they were entering Washington Street from the Mass Pike when they attempted to switch into the left lane. They described a large truck blocking their view so when they merged into the lane they hit MV2, and then turned and hit MV3. Operator of MV2 explained to me that they were on traveling Eastbound and was right behind a truck. She said she was following at the normal distance. MV1 then hit MV2. Operator of MV3 said that they were traveling in the center lane when they were hit by MV1. There was minor damage to MV1 front bumper on both sides. There was minor damage to MV2 right rear. There was minor damage to MV3 left rear. All parties were offered and declined medical attention. All vehicles were driven from the scene.
