Motor Vehicle 1 was traveling westbound on Aburndale Ave when it was struck in the rear drivers side by vehicle 2. Motor Vehicle 2 was stopped at the stop sign on Rowe St @ Auburndale Ave and proceeded to pull forward when it struck the rear drivers side of Motor Vehicle 1. Neither operators reported any injuries. Both vehicles only had very minor damage. No tows were required.

The operator of Vehicle #1 stated that as she was travelling Northbound on Pearl St, she collided with Vehicle #2. She stated that she didn't see it, prior to the collision. It should be noted that Vehicle #2 was parked, and unoccupied. Vehicle#1 was towed due to extensive damage. There were no injuries due to this accident.

Mv#1 operator stated she backed out of her driveway onto Crystal St and collided with Mv#2 that was travelling on Crystal St N/B. #1 driver's side rear struck #2 passenger side doors. #2 operator stated he was travelling on Crystal St N/B (right of way) and was struck by #1 which had suddenly backed out from the driveway.

MV#1 was traveling west on Beacon St. MV#2 was backing out of the driveway of 997 Beacon St. MV#1 crashed into MV#2. Operator of MV#1 stated that she did not see MV#2 backing out of the driveway when she crashed into him. Operator of MV#2 stated the same, that as he was backing out, he did not see MV#1 traveling on Beacon St. MV#1 was towed off scene by AAA. MV#2 was driven off scene. No injuries occurred.

Vehicle 1 was parked in marked lanes parking spot and shopping in Whole foods. Witness stated they saw vehicle 2 hit vehicle 1 causing damage. Vehicle 2 took off without exchanging paperwork. Witness stood by vehicle 1 till owner came out and stated their vehicle got hit. I attempted to contact vehicle 2 owner with negative results. On 1/07/2019 I made contact with owner of vehicle 2. Stated that she tapped vehicle 1 and then went to park her car in a designated parking spot to exchange paperwork when she then realized vehicle 1 had already left.

OPERATOR OF MV 1 STATED HE WAS TRAVELING NORTHBOUND ON PEARL ST AND AS HE APPROACHED WASHBURN STREET, MV 2 ENTERED THE INTERSECTION ATTEMPTING TO CROSS PEARL ST AND CONTINUE STRAIGHT ON WASHBURN STREET. MV 1 AND MV 2 COLLIDED WHICH RESULTED IN MV 2 STRIKING MV 3 WHICH WAS ALSO STOPPED AT THE OTHER STOP SIGN AT WASHBURN AND PEARL ST. MV 1 SUSTAINED FRONT END DAMAGE AND NO INJURIES REPORTED. MV 2 SUSTAINED MODERATE SIDE DAMAGE AND BOTH OCCUPANTS OF THE VEHICLE SIGNED PATIENT REFUSALS FROM MEDICS. MV 3 SUSTAINED FRONT END DAMAGE AND NO INJURIES REPORTED. OPERATOR OF MV 2 WAS ISSUED MA UNIFORM CITATION T1441116 FOR CH. 89 S. 9 FTC STOP SIGN.

Operator of MV1 states he was leaving the parking lot of 860 Walnut St, turning left, Southbound onto Walnut. He states he did not see MV2, which was turning left, Northbound on Walnut St from the Walgreens parking lot across the street. The two cars collided in the middle of the roadway. Operator of MV2 states he was leaving the Walgreens parking lot via the Walnut St exit. He states he never saw MV1 until they collided in the middle of the street head on. Both vehicles had airbag deployment. Newton PB3 obtained two patient refusals. Todys towed both vehicles, tow form filled out.

On Wednesday, January 9, 2019, while assigned to Traffic unit N525, I responded to 457 Walnut Street, Newton for a report of a past motor vehicle/pedestrian accident involving a student at Newton North High School. I met with the pedestrian involved in the accident, Ellie McBride, in the Riley House admin office. McBride is a Freshman at Newton North High School. McBride's mother, Linda Hanusaik, was also present while I spoke with her. McBride stated at approximately 0740 hours, she was attempting to cross Lowell Avenue in a marked crosswalk at Highland Street (E). McBride stated she observed a newer model gray Acura SUV traveling towards her Southbound on Lowell Avenue while she was standing in the crosswalk. McBride stated she thought the vehicle was going to slow down for her. McBride stated as the vehicle got closer it appeared to be slowing down but was unable to avoid making contact with her. The front end of MV1 made contact with McBride's left side. The impact caused McBride to fall down onto the roadway. McBride stated the vehicle pulled over and the operator exited the vehicle to check on her. McBride stated the operator was a white female with long brown hair, possibly high school aged. McBride stated after she told the operator that she wasn't hurt, the operator returned to her vehicle and left the area, possibly towards Commonwealth Avenue. McBride could not provide any additional information about the operator of the vehicle involved. McBride stated she then walked to Newton North High School. McBride stated she went to her morning class but left and texted her mother to tell her she was involved in an accident. McBride stated she was shaken up by the accident, but reported no injuries. Newton Medics evaluated McBride and obtained a patient refusal. McBride was advised that due to the reported age of operator, they may be a student at the high school. McBride was advised that if she sees the operator to obtain her information/contact NPD. I checked the area of Lowell Avenue and Highland Street for the vehicle involved/witnesses with a negative result. A portion of Newton North's athletic fields are next to this intersection. Newton North stated there are no surveillance cameras in this area. Lowell Avenue and Highland Street are both public ways maintained by the City of Newton.

On 01/09/2019, at 1806 hrs, I responded to Washington St. on the Mass Pike overpass for a two car MVA. Upon arrival, I observed MV1 to have damage to the front passenger side and MV2 to have damage to the drivers side. No injuries were reported. The area of Washington St where the accident occurred is five lanes across in the same direction. The operator of MV1 stated he was attempting to change lanes to his right when he collided with MV2. The operator of MV2 stated he was attempting to change lanes to his left when he collided with MV1.

The operator of MV#1 stated she was travelling northbound on Walnut St attempting to make a left turn onto Hull St when her front passenger mirror was struck by MV#2 who was travelling northbound on Walnut St. The operator of MV#1 stated the operator of MV#2 must have known their vehicles made contact because he stopped momentarily, looked at her and drove away at a high rate of speed. The operator of MV#2 was described as a white male in his 40's. The registration for MV#2 was 7KX977. There were no reported injuries to the operator of MV#1 or its passenger. There were no visible damages to MV#1. A query of MV#2 (MA REG 7KX977) showed that it is registered to a female party out of a Newton MA address. I was able to speak to the registered owner of MV#2 and she stated that her husband was driving her vehicle this morning and gave me his cell phone. I spoke to the husband (Operator of MV#2) and he verified that he was travelling northbound on Walnut St earlier this morning but was unaware that he had made contact with any other vehicles on the road. I examined the driver's side mirror of MV#2 and did not observe any visible damages. The operator of MV#2 was forthcoming with his statements. There were no reported injuries to the operator of MV#2. Based on the statements made to me I advised all parties involved that this accident will be documented.

On the above date and time I responded to the area of Washington Street at Centre Street for a two motor vehicle crash. Upon arrival I met with the operator of vehicle one Mr. Kramer, he stated he was traveling north on Centre St at Mass pike over pass bridge. He stated he thought the traffic signal was maybe yellow and he went through the intersection and struck vehicle two causing damage to the left rear bumper and left rear wheel. I spoke to the operator of vehicle two Mr. Bidwell, he stated he was stopped at the red light facing west on Washington Street mass pike off ramp. He stated the traffic signal turned green, as per his work policy he waited 3 full seconds before accelerating. He stated he proceeded through the intersection and was struck by vehicle one in the left side rear. Due to the left rear wheel being struck I notified Tody's towing of 1354 Washington Street arrived on scene and towed vehicle two to their tow lot. Neither party had any injuries and they were both advised. I checked the traffic signals in the intersection and they were fully functional.

On 01/11/2019, I responded to 1466 Chestnut St for a four vehicle accident. On arrival, I spoke with the operator of MV1 who stated that she was driving westbound on Commonwealth Ave and was unsure what happened but thought the vehicle in front of her came to a sudden stop. MV1 stated she attempted to stop but could not, and collided with the rear of MV2. The operator of MV2 stated that he was stopped on Commonwealth Ave behind MV3 when he was hit from behind by MV1 and pushed into the rear of MV3. The operator of MV3 stated she was stopped behind MV4 when he was rear ended by MV3, and pushed into the rear of MV4. The operator of MV4 stated that she was stopped on Commonwealth Ave due to traffic turning into the driveway of 1466 Commonwealth Ave. MV4 stated that she was stopped, with her left turn signal on for some time before she heard the sounds of a car accident behind her. MV1 sustained heavy front end damage and was towed by Tody's. MV2 sustained rear end and front end damage and was towed by AAA. MV3 sustained rear damage and minor front end damage. MV4 sustained minor scratches to the rear bumper. All parties involved declined medical attention.

MV1 was travelling SB on Centre St when they attempted to switch lanes and struck MV2. MV2 was travelling SB on Centre St in the right lane when she was struck by MV1. No injuries. MV1 towed by AAA due to being disabled with minor rear end damage. MV2 sustained minor front end damage and was towed due to the operator being unlicensed. Operator 2 had a international driving permit that expired on 6/12/18.

M/V #1 WAS TAKING A RIGHT TURN ONTO CENTRE ST WHEN IT STRUCK A TRAFFIC SIGNAL LIGHT ON THE SIDEWALK. THE LIGHT WAS KNOCKED OVER AND DAMAGED. TRAFFIC RESPONDED AND TOOK PHOTOS.

On 01/15/2019, at 1919 hrs, I responded to the intersection of Washington St and Centre St for a two car MVA. Upon arrival, I observed heavy damage to the front end of MV1 and moderate damage to the rear end of MV2. The operator of MV2 complained of head/neck injuries and was transported to Newton Wellesley Hospital by Cataldo for an evaluation. The operator of MV1 stated she was attempting to merge onto Washington St from Centre St when she rear ended MV2. The operator of MV2 stated when she slowed to merge onto Washington St from Centre St she was rear ended by MV1. Both vehicles were towed by Tody's. Towed MV inventory forms were filled out and filed.

On the above date/time I responded to Lexington Street / Freeman Street for a two motor vehicle crash with air bag deployment. Upon arrival on scene I checked all parties for injuries. Operator of vehicle one Ms. Farnese complained of head / neck injuries and Fire and Medics responded on scene. All parties in vehicle two did not have any injuries. I spoke with the operator of vehicle one Ms. Farnese in the ambulance she stated she was slowing traveling south on Lexington Street at the intersection of Freeman Street due to the traffic signal for Lexington Street was yellow. She stated as she was slowing she felt a heavy impact from in the rear by vehicle two. I spoke to the operator of two Ms. Heimbrock, she stated she is a pastor for the Salvation Army in New York and she was transporting two children Shamah Lapaix d.o.b. 4-18-14 and Grace Lapaix d.o.b. 1-10-11 to a Christian school in Wayland. She stated that she is staying with the children at 65 Falmouth Road Newton MA with the childrens grandmother due to the fact the parents are in New York doing a continuing education program until Friday January 18, 2019. She stated she is not familiar with the area, she was traveling south on Lexington Street approaching Freeman Street when she glanced at the map on her cell phone. She stated she did not observe the color of the traffic signal and did not observe the rear brake lights in vehicle one. All parties were evaluated in vehilce two by medics with no injury. I spoke with the childrens mother Sarah Lapaix 1-203-685-2809 who confirmed Ms. Heimbrock's statement about the care of the children. I did observe an expired inspection sticker as of 12/18 on vehicle one. Therefore due to operator of vehicle two's statement I checked the rear brake lights. When the brake pedal was pressed the left brake light and top(3rd) brake light was operational but the right rear brake light was not operational. Tody's towing arrived on scene and towed both vehicles top their lot. Ms. Heimbrock made arrangements for transport of herself and the children. Ms. Farnese was transported to Newton Wellesley Hospital for non life threatening injuries. I advised all parties of the process and gave them each others information. I checked the traffic signals in the intersection and they were fully functional. Therefore with all the above mentioned I am issuing citations to both parties Operator of vehicle one citation # T1440538 for Chapter 90 Section 20 EXPIRED INSPECTION STICKER $ 55.00 Operator of vehicle two citation # T1440539 for City Ordinance 19-75 FAILURE TO USE CARE IN STOPPING $ 20.00 Both citations were filled out after investigation and will be mailed to their license address.

ON 1-20-19 AT APPROX. 1344HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT THE INTERSECTION OF COMM AVE AND VALENTINE ST. I SPOKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES HE WAS TRAVELING S-BOUND ON VALENTINE AND STOPPED BEHIND VEHICLE #2. HE STATES WHILE STOPPED, VEHICLE #2 BEGAN BACKING UP. DRIVER OF VEHICLE #1 STATES HE BEEPED AND DESPITE HIS BEEPING VEHICLE #2 BACKED INTO VEHICLE #1 CAUSING EXTENSIVE FRONT END DAMAGE. I SPOKE TO THE OPERATOR OF VEHICLE #2. DRIVER STATES HE WAS STOPPED AT THE STOP SIGN AT VALENTINE AND COMM. AND BEGAN BACKING TO PLOW SOME SNOW. HE STATES HE DID NOT SEE VEHICLE #1 AND WAS NOT AWARE OF HIS PRESENCE UNTIL HE HAD BACKED INTO HIM. VEHICLE #1 HAD EXTENSIVE FRONT END DAMAGE AND WAS TOWED BY TODYS. VEHICLE #2 HAD NO DAMAGE TO HIS VEHICLE AND REPORTED NO INJURIES. VEHICLE #2 WAS PARKED ON THE CARRIAGE LANE WHILE DOMENIC ( A COSCO LANDSCAPE REPRESENTATIVE ) CAME TO RETRIEVE IT . THE OPERATOR OF VEHICLE #2 WAS ARRESTED ON AN UNRELATED WARRANT ISSUE ( DOCKET # 1861RO0190 ).

Operator of MV1 stated he was traveling Eastbound on Commonwealth ave in his lane of travel when he observed MV2 parked on the right hand portion on Commonwealth ave. Operator of MV1 stated as he was approaching MV2 he attempted to slow down in an effort to avoid colliding with MV2, but stated he could not stop due to the slippery surfaces on the road and collided with MV2. MV2 was parked and unoccupied at the time of the accident. I spoke to Owner of MV2 who shorty arrived on scene after the accident who stated he parked in front of 676 Commonwealth ave for a while to shovel snow in his driveway. No injuries. MV1 was towed by AAA and MV2 was driven off by owner to his nearby residence. MV1 sustained heavy right front end damage. MV2 sustained heavy rear end damage.

On 01/26/19 at 09:58 hours, I responded to a two car motor vehicle accident at Chestnut and Elliot Streets with no reported injuries. Both streets are public ways and maintained by the City of Newton. Operator of M/V # 1 (Quinn) states that she was stopped at the intersection of the above mentioned intersection going (EB) on Elliot Street. Once she received the green light signal she proceeded straight through and was struck by M/V # 2 on her passenger side. The vehicle sustained heavy damage to the passenger side front quarter panel area and it was towed by Tody's towing. Operator of M/V # 2 (Sanaguaray) states that he was operating straight ahead (NB) on Chestnut Street. He states that he thought the signal light was yellow when he proceeded through and does not recall what happened next. The vehicle sustained heavy front end damage and it was towed by Tody's towing. There were two witnesses to the accident. The first one, Karen Cherson, stated that she was traveling behind M/V # 2 when she observed him speeding and run through the red light striking M/V # 1. She said that the light was red by at least two seconds when M/V # 2 went through it. The second witness, John Quackenbush, states he was operating behind M/V # 1 when he observed their travel lane receive the green light signal. M/V # 1 then attempted to proceed through the intersection when it was struck on the passenger side by M/V # 2. Based on the two witness statements, it appears that Sanaguaray (M/V # 2) proceeded straight through the above mentioned intersection against the red light signal and struck M/V # 1. Sanaguaray was also operating a motor vehicle without a license and was issued a MA Uniform Citation # T1441052 for Ch. 89/9 and Ch. 90/10/A. He further stated that he has resided in Massachusetts for five years and was assigned a number of #S19552221 through the MA RMV but it shows no license status.

Veh #1 stated she was driving from Elliot St. to Rt 9 East ramp when she was stopped waiting to merge. She started and then stopped because it was not safe to proceed when veh #2 struck her from behind. Moderate damage to veh #1. Operator was checked out by Ambulance #1 for a complaint of neck pain and signed a patient refusal. Veh #2 stated she was also merging on Rt 9 East when veh #1 in front of her stopped and she didn't have time to. Light damage to veh #2. Operator of veh#2 was cited in hand Mass #T1441213 for N.C.O. 19/75-Failure to use care in stopping.

On Tuesday, January 29, 2019, while assigned to Traffic unit N525, I responded to the intersection of Cherry Street and Webster Street, Newton for a report of a motor vehicle accident involving a City of Newton owned vehicle. Cherry Street and Webster Street are both public ways maintained by the City of Newton. The weather at the time of the accident was overcast. The road surface was dry. The operator of MV1, John Bryant III (MA: S61611545), stated he was operating a City of Newton owned Volvo BL70 (MA MVN: M84236) Eastbound on Webster Street past Cherry Street. Bryant stated as he came to a stop in traffic, MV2 crashed into the rear of the vehicle he was operating. I observed minor damage to the rear bucket area of MV1. Bryant reported no injuries on scene and signed a patient refusal with Newton Medics. Bryant works for the Utilities Department with the DPW. The operator of MV2, Andrew Bell (NJ: B23900426706004), stated he was operating his 2013 Volkswagon (NJ: C54KKJ) Eastbound on Webster Street past Cherry Street. Bell stated he was following MV1 a little too close while they were traveling down Webster Street. Bell stated while traveling down Webster Street he looked down quickly but does not remember why, and when he looked up MV1 had come to a stop. Bell stated he could not avoid crashing into the rear of MV1. I observed significant damage to the front hood/windshield area of MV1. Bell was transported to Newton Wellesley Hospital with a minor injury. Bell was provided with Massachusetts Uniform Citation T1272991 in hand for Newton City Ordinance Chapter 19 Section 71 (Following Too Closely) . Pictures were taken of both vehicles and submitted to the IT Bureau.

Operator of vehicle 1 stated she was traveling westbound on Dedham St when vehicle 2 pulled out of Rachel Rd in front of her suddenly causing her to strike the passenger side of vehicle 2. Vehicle 1 then bounced to the right and went head on with a tree. Operator of vehicle 2 stated he stopped for the stop sign on Rachel Rd and was waived out onto Dedham St by another vehicle that was in front of vehicle 1. He then stated he pulled onto Dedham St and was struck by vehicle 1. A witness, Regina King was traveling westbound on Dedham St two car lengths behind vehicle 1 and saw vehicle 2 pull quickly out of Rachel Rd cutting off vehicle 1. King states that she did not see any other vehicle wave vehicle 2 onto Dedham St. I issued the operator of vehicle 2 MA Uniform Citation #T1272285 for 89/9 failure to comply with a stop sign. Vehicle 1 was towed by todys and operator of vehicle 1 was seen by the medics and signed a patient refusal. Vehicle 2 did not require a tow and the operator reported no injuries.

On 01/31/19 at 10:39 hours, I responded to the parking lot of the Jewish Community Center located at 333 Nahanton Street for a motor vehicle hit and run accident. The parking lot is accessible to the public and designed for vehicles to travel to and from the JCC. Owner of M/V 1 states she was parked on the side of the handicapped parking stall thinking it was ok to park there. She then ran in to the JCC for approximately one minute and when she returned she observed her driver's side front door was struck. I observed significant damage to the door frame and the door would not close. The owner called for a private tow to remove the vehicle. The staff at the JCC stated that a Martha Bixby gave her number and stated that she observed a black colored car with MA reg. # 934HH4 strike M/V 1. I attempted to call the number and left a message. The listing for that plate comes back to a 2010 Lexus ES350 color gray. I was then showed the video surveillance from the JCC's cameras and it shows an older white female exiting the lobby wearing a winter hat and carrying a bag with purple flower designs on it. She then walks through the lot and enters a gray Lexus ES350. The Lexus then starts backing out of the spot. The Lexus then continues backwards in to the other side of the parking stalls and strikes M/V 1 on the driver's side front door. The Lexus then pulls forward and leaves the scene without stopping or providing her information. As the Lexus is leaving, you can see there is a dent to the driver's side rear bumper. The video is not clear enough to see the plate on the Lexus or clearly see the operator's face. After, I located a number for the owner of the Lexus M/V 2 (Tania Lefman). I spoke to Lefman on the phone and she stated that she was backing out of the JCC parking lot at that time and felt a tiny bump to her rear end. She says that she could not stop in time due to the icy parking lot and then left the area thinking that nothing was wrong, it was only minor. I advised her there was significant damage to the other car. She was issued and mailed MA Uniform Citation T1441054 for Ch. 90/24/C Leaving the scene of property damage.

MV#1 Stated while traveling Westbound on Watertown St at the intersection of Eliot Ave his vehicle was struck on the driver side by MV#2. MV#1 stated MV#2 was attempting to cross Watertown St from Eddy St when MV#2 entered his lane of travel.MV#1 had heavy front and driver side damage following the collision. MV#2 Stated while traveling Northbound on Eddy St she attempted to cross Watertown St and on to Eliot Ave. MV#2 stated MV#1 had dim headlights and appeared to be traveling at a high rate of speed. MV#2 stated the front passenger side of her vehicle struck MV#1 on the driver side. Both MV#1 and MV#2 were towed from the scene due to heavy damage to both vehicles. All parties signed a medical refusal on scene and stated the were not injured at this time.

Operator of MV1 stated he was traveling Westbound on Washington St. when MV2 collided with his vehicle attempting to take a left traveling Eastbound on Washington St. Operator of MV1 stated there was a vehicle traveling parallel to him on Washington St. that stopped at the intersection of Washington St. and Hovey St. but stated he did not see MV2 until the last second. MV2 stated he was facing Southbound on Hovey St. attempting to take a left on Washington St. to travel Eastbound when he observed MV1 at the last second and collided with him. No injuries. MV1 sustained heavy front end damage. MV2 sustained heavy front left end damage along with drivers side door damage. Both vehicles involved were towed on scene by Tody's towing. A towed motor vehicle form was filled out for both in regards.

On 2/3/19, I was dispatched to 33 Austin St (Star Market) for an accident in the parking lot. On arrival I met with the operator of MV2 (MA Reg: 819VR6). MV2 stated that she was turning into the Star Market parking lot while someone was exiting the lot in the wrong direction. MV2 stated that due to the other vehicle going out the wrong way, she had to move over and collided with MV1. The operator of MV2 stated that her car came in contact with the drivers side front bumper of MV1. The operator of MV1 (MA Reg: 977GE4) was not on scene at the time of the accident. MV1 later arrived on scene and was appraised of what happened. MV1 was parked and unoccupied when it was hit by MV1. (cid:31)MV1 sustained damage to the passengers side. MV2 sustained damage to the front of the vehicle. Neither party was injured.

On 02/06/2019, at 1800 hrs, I responded to the area of 787 Washington St for a two car MVA with possible minor injuries. Upon arrival, I observed MV1 and MV2 both facing eastbound on the center double yellow line. It appeared that MV1 was in the outer lane and changed lanes, striking MV2 in the inner lane. MV1 had damage to the drivers side and MV2 had damage to the passenger side. The operator of MV1 stated he changed lanes and didn't see MV2 before they collided. The operator of MV2 stated MV2 abruptly changed into her lane and struck her. A witness corroborated the above statements. MV2 was towed by Tody's and both parties involved signed patient refusals with Cataldo.

MV1 stated she was travelling s/b on Crafts St when MV2 "t-boned me" causing damage to the passenger side. MV1 reported no injuries and called for a private tow. MV2 stated he was travelling n/b on Walnut St and "pulled out too far at the stop sign" subsequently striking MV1 causing minor damage to the front of their vehicle. MV2 reported no injuries, did not require a tow and was cited for 89/9 Stop Sign.

Owner of vehicle #1 reports that a waste management driver alerted her that he observed a blue F250 pickup truck back up from the driveway across from her and hit her vehicle on the rear drivers side. operator of suspect truck then pulled back into the driveway before backing out again heading east towards jasett st. witness gave possible ma registration of 969-wr4 but the plate didn't match the description of the vehicle. I will check with residents of property of #15 jasett to see if they know anyone who operates a blue F250. There was no answer at the door at this time. On February 10th, i went back to #15 jasett st and spoke with Jesse Grillo and told her about this accident. i asked her if she new anyone who drove a blue ford f250 that might have came to her home looking for her or her husband? She stated no.

OPMV#1 Was traveling Eastbound on Commonwealth Ave stopped in traffic when he was struck from behind by the OPMV#2. OPMV#1 Stated he was stopped in traffic and was suddenly rear-ended. OPMV#2 Was traveling Eastbound on Commonwealth Ave when he struck the OPMV#1 that was stopped in traffic. OPMV#2 Stated he was picking up his daughter and got distracted when he noticed her across the street and thats when he struck the vehicle in front of him.

MV#1 was traveling west on Homer St. MV#2 was traveling across Homer St, from Kenwood Ave south onto Pleasant St. Operator of MV#1 stated that she did not see MV#2 crossing Homer St. Operator of MV#1 stated that as MV#2 was crossing Homer St, he crashed into her. Operator of MV#2 stated that he did not see MV#1 traveling on Homer St as he attempted to cross. Operator of MV#2 stated that when he was crossing Homer St, he crashed into MV#1. MV#2 airbags deployed. MV#1 and MV#2 were towed off scene by Todys. Both parties signed patient refusals.

Operator 1 states he was traveling Eastbound on Beacon St and came to a complete stop for pedestrians in the crosswalk. While he was waiting he was struck from behind by vehicle 2. Operator 2 stated he was traveling Eastbound on Beacon St and didn't see vehicle 1 stop in front of him and he struck vehicle 1. MV 1 sustained minor damage and vehicle 2 sustained moderate damage and was towed by Tody's. There were no injuries.

On Monday, February 11th, 2019, I responded to a two vehicle MV accident in the parking garage at Lifetime Fitness Center at 300 Boylston St. There were no injuries. Vehicle # 1 (Bearing Mass Reg # 16KK69, Toyota Rav4, owned by Sasha Bordett) was unoccupied and parked in a marked spot facing in. Vehicle # 2 ( Bearing Mass Reg #875LD0, Toyota SUV, owned/operated by Cassandra Spittel) was parked backed in to the spot next to vehicle # 1. Cassandra stated she took a right hand turn out of the parking spot and heard a noise. She stopped her vehicle and inspected the damage and saw scratches to the right front side of her vehicle. She also noticed a dent to the right rear of Bordett's vehicle. Since the other vehicle was unoccupied, Cassandra called Newton Police and stated she had been in an accident and left her info to myself to make an accident report. I contacted the owner of the Toyota Rav4 and informed her of the accident and informed her their would be an accident report on file.

Based on observations and statements made, the following occoured. M/V#1 was traveling north on chestnut st., following the flow of traffic, when M/V#2 pulled out of a parking lot and collided with the left rear of M/V#1. The operator of M/V#2 stated that she just didn't see M/V#1.

V.1 Was stopped at the intersection of Walnut St and Comm Ave Southbound waiting for the traffic light to turn green when they were hit by V.2 in the rear of their vehicle. V.2 came to a complete stop behind V.1. When the left lane turn arrow turned green V.2 pulled forward a little and struck V.1 in the rear.There was no visible damage to either vehicle. Pictures were taken and turned into IT.

OPERATOR OF MV 1 STATED HE WAS TRAVELING SOUTH ON CRAFTS ST AND AS HE WAS PASSING THE ARS RESTORATION SPECIALISTS PARKING LOT WHICH WAS ON HIS RIGHT, HE WAS STRUCK BY MV 2 WHO WAS BACKING OUT OF THE PARKING LOT AND INTO TRAFFIC ON CRAFTS ST. MV 2 WAS PLOWING THE SNOW IN THE PARKING LOT AND AS HE WAS BACKING UP, HIS PASSENGER SIDE REAR BUMPER STRUCK MV 1 ON THE DRIVERS SIDE. BOTH VEHICLES HAD MODERATE DAMAGE AND NO INJURIES WERE REPORTED. I TOOK SEVERAL PHOTOS OF THE ACCIDENT AND PUT THEM IN IT'S BOX TO BE DOWNLOADED.

Mv#1 operator stated she was at a complete stop in traffic on Centre St N/B opposite #1269. #1 stated she was suddenly rear ended by Mv#2. No injuries reported. #1 appeared to sustain moderate damage and was driven away from the scene. #2 operator stated she was travelling straight ahead on Centre St N/B passing through the Beacon St intersection. #2 stated she was distracted by watching a pedestrian nearby and as a result rear ended #1 ahead of her. No injuries. #1 sustained heavy front damage and was towed by AAA towing.

The owner of MV#1 stated her vehicle was parked in the rear parking lot (Located on Rice St) of 1066 Centre St and was informed by 2 witnesses that stated her vehicle was struck by MV#2. I spoke to the 2 witnesses and they stated they were on the second floor of 1066 Centre St in suite #212 and had a bird's eye view of the rear parking lot on Rice St. The witnesses stated that MV#1 was parked while MV#2 was parked at an angle at a very close proximity to MV#1. The witnesses stated the operator of MV#2 was cleaning the snow off her vehicle, got into her vehicle then backed out of her parking space striking MV#1. The witnesses took a digital photograph of MV#2's registration (MA REG 763YC9). They described the operator of MV#2 as an Asian female in her 40's, 5'1" in height, operating a black colored "Buick" S.U.V type vehicle. MV#1 sustained minor damages to its front passenger door and bumper. A query of MA REG 763YC9 showed that it was registered to an Asian female to a black colored "Buick Rendezvous". The owner of MV#1 stated the owner of MV#2 also has a business in the same building at 630 Commonwealth Ave (Move2Boston Group). I spoke to the registered owner of MV#2 via telephone and she stated she was parked in the rear parking lot on Rice St but denied making contact with any vehicles. I could not observe MV#2 as it was not at the scene. At 14:40 I returned to 1066 Centre St and was able to observe MV#2. MV#2 displayed minor scrapes to its front driver's side bumper which was consistent relative to where both vehicles were parked. Based on the witnesses statements I will document this accident and provide this report number to the vehicle owners.

OPERATOR OF MV 1 STATED HE WAS TRAVELING NORTH OFF OF THE RT 95 EXIT RAMP (EXIT 24) AT COMMONWEALTH AVE WHEN HE CAME TO A STOP BEFORE HE TURNED RIGHT ONTO COMMONWEALTH AVE. HE WAS STRUCK IN THE REAR BY MV 2 WHO WAS BEHIND HIM CAUSING MINOR DAMAGE TO HIS REAR BUMPER AND NO INJURIES WERE REPORTED. OPERATOR OF MV 2 WAS TRAVELING BEHIND MV 1 AND STRUCK MV 1 UIN THE REAR WHEN HE CAME TO A STOP. MINOR DAMAGE SUSTAINED AND NO INJURIES WERE REPORTED.

Operator of vehicle 1 stated she was stopped at the stop sign on Crescent St southbound @ Beacon St and proceeded forward and struck the passenger side of vehicle 2. Operator of vehicle 2 stated she was traveling westbound when vehicle 1 pulled out of Crescent St and struck her passenger side. Neither operator reported any injuries. Both vehicles were towed by Todys towing.

The operator of MV1 (ma con T98504) was traveling WB on Washington Street attempting to take a left into 300 Washington Street when his MV was rear ended. Operator of MV stated he was letting a vehicle exit from 300 Washington Street and had come to a complete stop when his vehicle was struck from behind. The operator of MV2 (ma con T12376) stated he was traveling WB on Washington Street and was unable to come to a complete stop before striking the rear of MV1. Air bags were deployed in MV2. The operator of MV2 stated he felt fine. Ambulance two and Ladder 3 were dispatched to the scene to evaluate the operator. Operator of MV2 signed a refusal. The other parties involved were uninjured. Tody's was also called on scene to tow MV2 as it sustained heavy front end damage and was not drive able. MV1 sustained minor damage to rear door and bumper but was able to operator without any issues.

On 02/26/2019, at 1852 hrs, I responded to the intersection of Washington St and Walnut St for a four car MVA. Upon arrival, I observed all four vehicles in the inner westbound lane of Washington St. All four vehicles sustained damage from a chain reaction rear end accident caused by MV1 in the rear. MV1 sustained heavy front end damage and was towed by Tody's. MV2 and MV3 sustained damage to their front and rear ends. MV4 sustained damage to its rear end. I spoke with the driver of MV1 who stated that he was distraught about family issues and looked away from the road before rear ending MV2. The other three drivers all stated that they were slowing or stopped at the red light when they were rear ended, corroborating the driver of MV1's statement. No injuries were reported and Cataldo received patient refusals.

ON 3-3-19 AT APPROX. 1223HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT THE INTERSECTION OF CRAFT ST AND ALBEMARLE RD. I SPOKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES SHE WAS HEADING S-BOUND ON CRAFT ST. STOPPED IN TRAFFIC PREPARING TO TAKE A LEFT ONTO ALBEMARLE RD. WHEN SHE WAS HIT IN THE REAR BY VEHICLE #2. I LATER INFORMED VEHICLE #1 THAT THE LEFT TURN ONTO ALBEMARLE FROM CRAFT IS PROHIBITED. VEHICLE #2 STATES SHE WAS TRAVELING S-BOUND ON CRAFT WHEN VEHICLE #1 STOPPED ABRUPTLY AND SHE WAS UNABLE TO AVOID HITTING HER. VEHICLE #1 HAD LEFT REAR END DAMAGE AND WAS OPERATIONAL. VEHICLE #2 HAD EXTENSIVE LEFT FRONT DAMAGE AND WAS TOWED BY TODYS. ALL PARTIES OF VEHICLE #1 REPORTED NO INJURIES. VEHICLE #2 HAD COMPLAINTS OF HEAD PAIN AND A CRACK IN THE WINDSHIELD FROM WHERE HER HEAD HAD HIT IT. OPERATOR OF VEHICLE #2 WAS TRANSPORTED TO NEWTON WELLESLEY HOSPITAL BY MEDIC2. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES.

On the above date/time I responded to the area of Washington Street/Centre St for a report crash involving two motor vehicles. Upon arrival I met the operator of v1, she stated she was traveling straight (east) when vehicle two crossed over lanes and struck her vehicle in the right rear. I spoke with the operator of v2, he stating he was merging lanes and did not observe vehicle one until impact. He stated the left front of his bumper had contact with vehicle two right rear. No reported injuries, all parties advised,all information was swapped.

OPERATOR OF MV 1 STATED SHE WAS TRAVELING NORTHBOUND ON CENTRE STREET AND WAS SUDDENLY REAR ENDED BY MV 2 WHO WAS TRAVELING BEHIND MV 1. MODERATE DAMAGE SUSTAINED TO THE REAR BUMPER OF MV 1 AND NO INJURIES WERE REPORTED. THE VEHICLE WAS TOWED PRIVATELY BY PERFECTION. OPERATOR OF MV 2 STATED SHE WAS TRAVELING BEHIND MV 1 AND ADMITTED TO CRASHING INTO MV 1'S BUMPER. MODERATE DAMAGE SUSTAINED TO MV 2 AND NO INJURIES WERE REPORTED.

MV1 stopped short to allow an unidentified MV back of of a parking space. He stopped behind MV2. MV2 did not see MV1 stop and backed into MV1.

On 03/08/2019, at 1714 hrs, I responded to 275 Centre St, Bertucci's parking lot, for a two car MVA. Upon arrival, I observed MV1 to have no damage to the rear end where she collided with MV2. MV2 had damage to the front passenger side door. The operator of MV1 stated that she did not see MV2 while she attempted to back out of a parking spot and collided with her passenger side. The operator of MV2 corroborated the story and stated she was attempting to exit the parking lot when MV1 backed into her passenger side.

On 03/09/2019, at 0117 hrs, I responded to Washington St. at Church St. for a two car MVA. Upon arrival, I observed both vehicles facing the same direction in the outer eastbound lane of Washington St. MV1 had moderate damage to the front end and MV2 had heavy damage to the rear end. The operator of MV1 stated that as he was approaching MV2 he attempted to slow down but the vehicle accelerated, rear ending MV2. The operator of MV1 stated that moments after the traffic light turned from red to green, MV2 rear ended her. The passenger in MV2 sustained a bruised lip but all involved parties signed patient refusals with Cataldo. Tody's towed MV2.

OPERATOR OF MV 1 STATED HE WAS TRAVELING SOUTHBOUND ON CRAFTS STREET AND AS HE APPROACHED THE INTERSECTION AT ALBERMARLE ROAD HIS VEHICLE WAS STRUCK ON THE DRIVERS SIDE BY MV 2. MODERATE DAMAGE SUSTAINED TO MV 1 AND NO INJURIES WERE REPORTED. MV 1 WAS TOWED BY TODYS TO THEIR LOT AFTER ALL VALUABLES WERE REMOVED BY THE OWNER. OPERATOR OF MV 2 WAS TRAVELING WESTBOUND ON ALBERMARLE ROAD AND AS HE DROVE ACROSS THE INTERSECTION OF CRAFTS STREET AFTER COMING TO A STOP AT THE STOP SIGN, HE STRUCK MV 1 ON THE DRIVERS SIDE. HEAVY DAMAGE SUSTAINED TO MV 2 AND OPERATOR WAS TRANSPORTED BY MEDICS TO NWH TO BE EVALUATED FOR HIS INJURIES. OPERATOR OF MV 2 WAS UNABLE TO PROVIDE A STATEMENT OF HOW THE ACCIDENT OCCURRED AND HIS VEHICLE WAS ALSO TOWED BY TODYS TO THEIR LOT. A WITNESS TO THE ACCIDENT IDENTIFIED AS BRIAN ABDOW STATED THAT MV 2 WAS AT FAULT. HE STATED THAT MV 2 CAME TO A STOP AT THE STOP SIGN BUT THEN ACCELERATED INTO TRAFFIC ON CRAFTS STREET AND STRUCK MV 1 WHO HAD THE RIGHT OF WAY. OPERATOR OF MV 2 IS BEING ISSUED MA UNIFORM CITATION T1268369 FOR CH.89 S.9 FAILURE TO COMPLY WITH STOP SIGN.

Mv#1 operator was on Beacon St E/B waiting on a green light to turn on Chestnut N/B. Mv#2 operator was directly behind #1, proceeded straight ahead when the light turned green and rear ended #1. #1 stated she may have been injured without specifying where she may have been hurt other than experiencing a headache. Medics responded and a patient refusal was signed. Both vehicles sustained minor damage, rear end and front end, respectively.

Page Davis states that she pulled over to the side of the road on Lowell Ave for an oncoming ambulance. Davis states that she looked in her mirror to enter the traffic lane. Davis saw vehicle #1 a distance back. Davis then went to enter the traffic lane and she was struck in the left rear by vehicle #1. Hanbo Lin was operating vehicle #1. Lin states that he saw vehicle #2 attempt to merge into traffic. Lin went to step on the brake and his vehicle slid into Davis due to the weather/road conditions. No injuries.

M/V 1 TRAVELING WEST ON WASHINGTON ST APPROACHING 897 WASHINGTON ST. M/V 2 WAS EXITING DRIVEWAY ONTO WASHINGTON ST. M/V 1 HAD RIGHT OF WAY AND M/V2 STRUCK M/V 1 , NO INJURIES .M/V 3 IN DIAGRAM IS A PARKED VEHICLE. M/V 2 WAS TOWED AWAY BY TODY'S DUE TO EXCESSIVE DAMAGE.

On 03/12/2019, I responded to the area of 279 Chestnut St for two car motor vehicle accident. On arrival, I spoke with the operator of MV1 (MA Reg: 824WA6) who stated that he was driving northbound on Chestnut St when he was rear ended by MV2. I spoke with the operator of MV2 (MA Reg: 4DN621) who stated that she was travelling northbound on Chestnut St behind MV1. MV2 stated that they were in stop and go traffic and misjudged the distance and rear ended MV1. MV1 sustained moderate damage to its rear end. MV2 sustained moderate damage to the front end and had airbags deployed. All parties were evaluated by medics and signed patient refusals. MV2 was towed from the scene by Tody's Towing.

MV1 had just picked up a student at 56 Crescent Ave(at the intersection of Throwbridge). He made the pick up just west of the stop sign. He turned off his illuminated bus stop sign, paused and then began to pull into traffic. MV2 was stopped at the stop sign on Trowbridge st at Crescent St waiting for the bus to turn off the stop sign. When the bus turned it off, it appeared to him that the bus was not pulling into traffic. MV2 began to turn left "very slowly". As he was driving next to the van, the van pulled into traffic and the 2 MVs touched mirrors. There was no damage to either MV. A BLS ambulance was called to the scene because there were three students on board. All parents showed up and signed refusals for their children. Pictures were taken of the two mirrors that had no damage. The following parents showed up on scene to sign refusals for their children: Gareth Gorman for Sophia 617-515-1576 Lori for Harry Melchionno 617-527-7751 Malagros Falu for Nathalie Cedno The students belong to the Oak Hill Middle school and they were notified.

OPERATOR OF MV 1 STATED HE WAS TRAVELING WESTBOUND ON NEWTONVILLE AVE AND CAME TO A COMPLETE STOP AT THE STOP SIGN BY WALNUT ST. MV 1 STATED HE WAS THEN REAR ENDED BY MV 2. OBSERVED NO DAMAGE TO MV 1 AND NO INJURIES REPORTED. OPERATOR OF MV 1 WAS ADAMANT THAT MV 2'S FRONT WINDSHIELD WAS COVERED IN ICE AT THE TIME OF THE ACCIDENT. OPERATOR OF MV 2 STATED SHE WAS TRAVELING WESTBOUND ON NEWTONVILLE AVE AND COULDN'T STOP IN TIME WHEN MV 1 STOPPED AT THE STOP SIGN IN FRONT OF HER. MV 1 STATED SHE BARELY TAPPED MV 1. I OBSERVED NO DAMAGE TO EITHER VEHICLE AND NO INJURIES WERE REPORTED.

The OP. of MV#1 states she had gotten off Rt-128N and was stopped waiting to merge onto Washington Street eastbound. She was then suddenly rear ended by MV#2. The OP. of MV#2 states he was initially behind MV#1 and thought MV#1 had already merged into traffic. When he saw a gap in traffic on Washington Street, he proceeded forward and collided into the rear of MV#1. I observed very minor damage, paint transfer and scratches. The OP. of MV#1 signed a refusal with the medics and will seek her own medical attention.

Operator of vehicle #1 stated he was travelling Westbound on Crafts St. and stated he became distracted by a noise coming from the left front wheel or axel. Operator #1 stated he took his eyes off the road and drifted to the right striking a utility pole. Operator # 1 had only a Guatemala ID card. Operator # 1 was cited for Unlicensed operation citation #T1268256.

On 03/16/2019, at 1652 hrs, I responded to the intersection of Newtonville Ave and Harvard St for a two car MVA. Upon arrival, I observed damage to the front drivers side of MV1 and damage to the front passenger side of MV2. The operator of MV1 stated she had a green light and was struck by MV2. The operator of MV2 stated she had a green light and was struck by MV1. Two witnesses stated that Newtonville Ave had a red light and MV2 was proceeding through a green light. Neither witness provided their personal information.

Operator of MV1 stated he had got off the Mass Pike at exit 17, heading Eastbound on Washington Street. As he was heading Eastbound, operating of MV 1 stated MV2 crossed lanes and struck the passenger door. Operator of MV2 stated while traveling Eastbound on Washington Street, she was in the 2nd lane from the left, heading toward St. James Street when MV1 struck the driver side of the vehicle. MV1 had minor damage to the passenger door. MV2 had significant damage to the driver side front and rear door, along with a flat front driver side tire. Both operators were checked by the medics and signed refusals. MV2 was towed by Tody's towing.

The OP. of MV#1 states she was stopped on the off-ramp of Rt-128N to merge onto Washington Street eastbound when she was suddenly struck in the rear by MV#2. The OP. of states he was behind MV#1 previously and thought MV#1 had already merged into Washington street traffic. While watching traffic to his left, there was an opening so he drove forward and collided into MV#1.

MV1 stated she was stopped on Washington St attempting to turn left onto Church St when she was rear ended by MV2 causing minor damage. MV1 did not require a tow, was evaluated and cleared by Cataldo Ambulance and departed the scene. MV2 stated she was "trying to get in the right lane but couldn't and when I came back I hit that car" causing minor damage. The operator of MV2 complained of pain to her head from striking the windshield and was transported by Cataldo to Newton Wellesley Hospital for an evaluation. Her vehicle was left parked legally on Washington St.

On 03/19/19 at 14:57 hours, I responded to 220 Dedham Street for a past motor vehicle hit and run accident involving a parked car that occurred at 12:26 hours on this date. Dedham Street is a public way maintained by the City of Newton. The owner of M/V # 1 states her vehicle was legally parked in front of 220 Dedham Street facing northbound when she came out and found a note from a witness saying it was struck by a gray Honda Accord bearing MA Reg. # 8512HV. It appears that the operator of M/V # 2 side swiped M/V # 1 as it was passing by on Dedham Street. I observed marks to the driver's side rear door area and the side view mirror was broken. On 03/20/19 at 08:47 hours, I spoke to the witness, Kawtar Bhih, on the phone and she stated that she was parked on the opposite side of where the accident took place. Bhih observed a gray Honda Accord with an elderly female operating M/V # 2 on Dedham Street almost side swiped another parked car before side swiping M/V # 1. She says that the vehicle slowed down after and then continued to leave the scene of the accident. She then took a photo of the above listed registration plate. I attempted to contact the owner of M/V # 2 (Jali Medical Inc Waltham, MA) and the receptionist was not able to provide any information at the time. I was told to call back the next day. I gave the vehicle and registration info to the person inquiring if they could find a possible driver and call back. While working on this report, I observed that the owner of M/V # 2 was involved in a past accident in our system and the operator at the time had an address of 66 Country Club Rd. Newton. On 03/20/19 at approx. 09:30 hours, I went over to this address and observed M/V # 2 in the driveway with an operator inside. After inquiring about the accident, she stated she was driving down Dedham Street when she heard a noise coming from her passenger side view mirror as she had struck something. She then left the scene not thinking it was serious. I observed fresh damage to her passenger side mirror along with other marks along the doors which appeared to be older. Operator was issued in hand MA Uniform Citation # T1441710 for Ch. 90/S24 Leaving the scene of property damage.

The operator of vehicle number 1 stated he was unable to brake in time to stop his vehicle from colliding with vehicle number 2 which was stopped in traffic.Vehicle number 1 struck the rear of vehicle number 2 The operator of vehicle number 2 stated he was stopped at the red light(just past woodland golf club), when he was rear ended by vehicle number 1. After being rear ended by vehicle number 1, vehicle number 2 was pushed into vehicle number 3. The operator of vehicle number 3 stated she was stopped at a red light when she was struck in the rear by vehicle number 2. vehicle number 1 sustained minor front end damage vehicle number 2 sustained moderate rear and damage and minor front end damage vehicle number 3 sustained minor rear end damage

MV#1 was stopped at the red light, while traveling south on Walnut St. MV#2 was attempting to turn left onto the north side of Walnut St from Centre St. MV#2 crashed into MV#1 on the southbound side of Centre St. Operator of MV#2 stated that she made the turn to soon, and crashed into MV#1. Operator of MV#1 stated the same information. No injuries occurred. MV#2 was driven off scene. MV#1 was towed off scene by AAA.

OPERATOR #1 STATED SHE WAS ON WATERTOWN ST IN FRONT OF ANTOINES BAKERY, BACKING UP INTO A PARKING SPACE SHE THOUGHT WAS OPEN WHEN SHE ACCIDENTLY BACKED INTO VEHICLE(2) WHICH WAS PARKED. OWNER OF VEHICLE #2 WAS STANDING OUTSIDE HER PARKED VEHICLE IN FRONT OF #317 WATERTOWN ST, ANTOINES BAKERY WHEN VEHICLE #1 BACKED INTO HER CAR.

MV1 was stopped in traffic waiting to turn left into the Cypress St Municipal lot when she was struck from behind by MV2. Operator 2 states she did not see MV1 stopped and bumped her from behind. No injuries to eithier parties, Newton Ambulance 1 obtained medical refusals. MV1 sustained minor rear end damage and was driven from the scene. MV2 sustained minor front end damage and had to be towed due to air bad deployment.

MV1, a City of Newton contracted school van, was the first car stopped at the red light north on Walnut St at Homer St. MV2 was slowing as she approached the red light and rear ended MV1. According to MV1, driver of MV2 got out of her MV and looked at both MVs. She then got back into her car and when the light turned green they both pulled over just past the intersection. MV2 asked MV1 if she hit him and he stated yes. She stated that she did not see his van, which is bright blue. MV2 operator stated to me that she was stopped at the traffic light and thought MV1 backed into her. When I told her MV1 statement she didn't argue. There was minimal damage to MV1. The outline of MV2 license plate was imprinted in dust on the bumper. There was no visible damage to MV2. I took pictures of the damage to MV1 and MV2. The following parents responded to sign refusals for their children: Wenlei Zhou for Sohia Zhou Katie Thatcher for Tristan Thatcher Both students went to school. The students go to the Newton Early Childhood school on Jackson Rd.

On 03/26/19 at 14:38 hours, I responded to 207 Nahanton Street for a two car motor vehicle accident with no injuries. Operator of M/V # 1 states she was traveling straight ahead westbound on Nahanton Street in stop and go heavy traffic. She states that she was not able to stop in time and struck M/V # 2 in the rear end. Her vehicle sustained damage to the front end bumper and grille area. Operator of M/V # 2 states he was stopped in traffic on Nahanton Street (W B) when he was rear ended by M/V # 1. His vehicle sustained rear end damage to the bumper, trunk area, and quarter panels.

MV1 was parked on Adams St and was sideswiped by MV2 causing minor damage. MV2 slowed down, however when approached by a witness fled the scene. MV2 is described as a blue sedan (possibly Toyota) with its passenger side mirror missing. A canvass of the area was conducted yielding negative results.

On Thursday, March 28, 2019 while assigned to Traffic unit N525, I responded to the area of Dedham Street and Bound Brook Road, Newton for a report of a motor vehicle accident. Dedham Street and Bound Brook Road are both public ways maintained by the City of Newton. Anna Johnson (S14645384) stated her vehicle, a 2012 Honda CRV (MA: 4AG988), was parked unoccupied on Dedham Street (S) right before Bound Brook Road. Johnson stated she was made aware that her vehicle was hit by a passing vehicle. When she arrived at her vehicle, she observed damage to the driver side door mirror and the rear driver side door area. A note with a name and phone number was left on her vehicle by the operator of MV2. I spoke with Maria Diaz-Santos (S91121221) stated she was operating her 2018 Chrysler Pacifica (MA: 1MMB61) south on Dedham Street towards Bound Brook Road. Diaz-Santos stated as she was traveling past MV1, her rear passenger side door mirror area made contact with the driver side of the parked vehicle. Diaz-Santos stated she contacted Newton PD to make them aware of the accident and left her name and phone number on MV1 for it's owner to exchange information. Diaz-Santos stated there is very minor damage to her passenger side door mirror.

Operator of MV1 stated she was stopped at a red light facing Westbound at the intersection of Commonwealth ave at Hammond St. when she was rear ended by MV2. The operator of MV2 stated she was traveling Westbound on Commonwealth ave when she attempted to break at the intersection of Commonwealth ave at Hammond St. but couldn't because her brakes failed colliding with MV1. MV1 sustained moderate rear end damage. MV2 sustained moderate front end damage. No injuries. MV2 was towed by Tody's on scene and a towed motor vehicle form was filled out in regards.

On 04/01/2019, at 1736 hrs, I responded to the intersection of Washington St and Waverley Ave for a single vehicle MVA. Upon arrival, I observed a city sign with street names and a stop sign bent over to the ground. There were tire marks across the small island leading up to the signs at the intersection. The vehicle on scene had damage to its front passenger side. The operator of the vehicle stated he was traveling westbound on Washington St and attempted to turn left onto Waverley Ave when he ran over the island and struck the sign. He stated a vehicle was traveling eastbound on Washington St which sped up and caused him to crash. Pictures were taken of the city property damage and submitted to IT. The city was also notified to replace the signs.

ON 4-6-19 AT APPROX. 1241HRS. WILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT 434 WATERTOWN ST. I SPOKE TO THE OWNER OF VEHICLE #1. OWNER STATES SHE WAS IN 334 WATERTOWN ST ( METRO CREDIT UNION ). SHE LOOKED OUT THE WINDOW AND SAW HER CAR SHAKE. SHE WENT OUTSIDE AND SAW HER CAR SHAKE AGAIN. SHE CONFRONTED THE OPERATOR OF VEHICLE #2 AND ADVISED HER THAT HE HIT HER VEHICLE. VEHICLE #2 YELLED AT HER DENYING HITTING HER CAR AND LEFT THE AREA. VEHICLE #1 HAD MINOR FRONT END SCRAPES. VEHICLE #1 REPORTED NO INJURIES AND WAS ADVISED TO CONTACT HER INSURANCE COMPANY. ATTEMPTS TO CONTACT VEHICLE #2 WERE UNSUCCESSFUL. VOICE MAIL LEFT. WILL ATTEMPT TO MAKE CONTACT ON NEXT SHIFT. ON 4-8-19 AT APPROX. 1035HRS. WHILE WORKING N492 I ATTEMPTED TO CONTACT THE OWNER OF VEHICLE #2. I LEFT ANOTHER VOICE MAIL. AS OF 4-9-19 1100HRS. AS I WRITE THIS REPORT I HAVE NOT RECEIVED A CALL BACK. VOICE MAIL ALSO ADVISED HE CONTACT HIS INSURANCE COMPANY FOR MORE INFORMATION.

On 04/07/2019, at 1749 hrs, I responded to the intersection of Saint James St and Washington St for a two car MVA. Upon arrival, I observed MV1 to have damage to its front end and MV2 to have damage to its rear end caused by a rear end accident. The operator of MV1 stated she was looking over her shoulder while merging onto Washington St and rear ended MV2. The operator of MV2 stated she slowed as she was merging onto Washington St when she was rear ended by MV1. No injuries were reported and both vehicles were driven from the scene.

Brian Feeley states that he was driving E/B on Beacon St. Feeley states that he struck a tree branch that was overhanging onto the street. Damage to the top right of the box truck. No injuries, no tows.

OWNER OF VEHICLE #1 REPORTS HE HAD HIS VEHICLE, 2011 BLACK DODGE CHALLENGER, MA REG 3NR 117 PARKED IN FRONT OF HIS GIRLFRIENDS HOUSE, #11 MELBOURNE AVE WHEN HE HEARD A LOUD CRASH FROM INSIDE. HE LOOKED OUTSIDE AND OBSERVED A LARGE WHITE BOX TRUCK LEAVING THE SCENE. NO FURTHER DESCRIPTION VEHICLE #2 LEFT SCENE AFTER CRASH. DUE TO CALIFORNIA ST BEING CLOSED GOING E/B ALL TRAFFIC WAS BEING DIVERTED DOWN MELBOURNE MAKING IT HAZARDOUS FOR CARS PARKED ON THE STREET.

OPERATOR OF MV 1 STATED HE WAS TRAVELING NORTHBOUND ON WALNUT ST AND WASN'T PAYING ATTENTION. HE STRUCK MV 2 FROM BEHIND WHO WAS STOPPED IN TRAFFIC IN FRONT OF HIM. DUE TO THE IMPACT OF THE CRASH, MV 2 REAR ENDED MV 3. THERE WERE NO INJURIES REPORTED (5 PATIENT REFUSALS SIGNED) AND MODERATE DAMAGE SUSTAINED TO ALL THREE VEHICLES, FRONT END DAMAGE TO M/V 2. MV'S 1 AND 2 WERE TOWED FROM THE SCENE BY TODYS TOWING TO THEIR LOT IN WEST NEWTON SQUARE AFTER ALL VALUABLES WERE SECURED FROM THE VEHICLES. OPERATOR OF MV 1 WAS CITED MASS UNIFORM CITATION T1268379 FOR C.O. 19/75 FAILURE TO USE CARE.

MV1 stated he was attempting to "turn left" when the car to his left "went straight instead of left" subsequently striking his vehicle. MV1 was spun sideways and MV2 continued into Watertown. I then made contact with the operator of MV2 who admitted to striking MV1, however thought they weren't going to stop so he kept going as well. MV2 was cited for Leaving the Scene of Property Damage & Marked Lanes See incident report 19014042 for further information.

OPERATOR#1 STATED HE WAS STOPPED AT THE OFF RAMP TO THE MASS PIKE E/B ATTEMPTING TO ENTER OUT ONTO CENTRE AVE WHEN HE WAS REAR ENDED BY VEHICLE #2. OPERATOR #2 STATED HE WAS STOPPED BEHIND VEHICLE #1 AT THE OFF RAMP TO THE MASS PIKE E/B. HE THEN REPORTS VEHICLE#1 BEGAN TO PULL OUT ONTO CENTRE AVE, WHEN HE TO BEGAN TO FOLLOW WHILE AT THE SAME TIME LOOKING TO HIS LEFT FOR ANY TRAFFIC COMING OVER THE BRIDGE. AS HE WAS LOOKING TO HIS LEFT VEHICLE #1 HAD STOPPED AGAIN CAUSING HIM TO CRASH INTO THE REAR OF HIS VEHICLE.

Mv#1 (truck) operator with Mv#2 (car trailer attached) was attempting to turn around on the dead end cul de sac on Troy Lane. The passenger side rear end of the trailer struck the vinyl fence abutting the back yard property owner of 41A Helene Rd. Damage appears minor to the end lower support post area and back yard birm between the fence and curbing.

The operator of MV1 stated that she was traveling southbound from Quinobequin Road onto Ellis Street when the right rear of her vehicle was struck by MV2 when they failed to stop at a stop sign. The operator of MV2 states that she was traveling Eastbound coming off of Boylston Street when she failed to see the posted stop sign subsequently striking MV1. MV1 sustained major damage to the right rear wheel and airbag deployment and was towed from the scene by Todys. MV2 sustained moderate damage to the front bumper but was driven from the scene. Medics evaluated both parties and they signed patient refusals. The operator of MV2 was giving in hand MA uniform citation T1443935 for 89-9 failure to stop.

MV#1 was parked outside 719 Washington St (was not occupied at the time) when MV#2 attempted to park in front of MV#1. MV#2 side swiped MV#1 on the front driver side causing moderate damage to MV#1. MV#1 was Towed at the request of the owner, Tody's took posetion of the vehicle. MV#2 stated he did not sustain any injuries at this time.

MV#1 stated she was traveling Northbound on Centre St. MV#1 stated she had took her eyes off the road momentarily when her vehicle drifted off to the right striking the sidewalk. MV#1 stated she then struck the telephone pole (Verizon 73/21) MV#1 sustained major damage to the front end rendering the vehicle completely disabled. Todys's took possession of the vehicle. Operator of MV#1 was taken to NWH for injuries sustained by medics.

Operator of vehicle 1 states he was driving eastbound on Elliot St when vehicle 2 backed out of the driveway of 166 Elliot St and they crashed. Operator of vehicle 2 stated he was backing out of the driveway of 166 Elliot St and was trying to turn to head westbound when he was struck by vehicle 1. All parties were evaluated by Cataldo medics and all signed refusals. Both vehicles were towed by Todys Towing.

Operator of vehicle one stated that while stopped in traffic north bound on Walnut St near number 27 her vehicle was rear ended by vehicle one. Operator of vehicle two stated that while driving north bound on Walnut St near number 27 his foot came off the clutch pedal causing his vehicle to rear end vehicle one. Neither vehicle had damage and both vehicles did not require tows. Neither operator stated that they were injured. I took photos of both vehicles and forward the disk to the NPD's IT bureau. I cited the operator of vehicle two for a violation of City of Newton Ordnance 19/71 , following to close. Walnut St is a public way in the city of Newton.

MV#1 was parked on the side of the road, in front of 38 Tanglewood Rd. MV#2 was traveling east on Tanglewood Rd, and turning North, continuing on Tanglewood Rd. MV#2 crashed into MV#1. Owner of MV#1 stated that she was not in the vehicle at the time. She stated that the operator of MV#2 came and found her to tell her that she had crashed into her. Operator of MV#2 stated that she was turning from east to north on Tanglewood Rd. Operator MV#2 stated that she did not see MV#1 parked on the side of the road, and crashed into it. MV#1 was towed by AAA. MV#2 was driven off scene. No injuries occurred.

Operator of vehicle 1 stated he was coming driving on Wolcott St when he vehicle caught the overhead power line going from poll to house #382. As a result the power line was pulled from the residence and came down across vehicle 2. Vehicle 2 had minor damage and the residence had damage from the line being pulled. Eversource responded and cut power to the line before safely removing it from the vehicles/roadway. No reported injuries and no tows were required.

Operator of vehicle 1 states she was was driving southbound on Grove St when a squirrell ran out infront of her. She swerved right to avoid the squirrell and hit a utility poll. Operator 1 was checked on by Cataldo medics and signed a patient refusal. The vehicle was towed by AAA and utilites were notified about possible damage to the poll.

Patrice Lys was operating vehicle #2 on Brandies Rd. Lys states that vehicle #1 was about to back out of the driveway so he stopped. Lys state that the vehicle did not back out so he started to drive again, at that point vehicle #1 backed out of the driveway and struck his vehicle on the left rear. Azad Chitchian was operating vehicle #1. Azad states that she saw vehicle #2 stop to let her out of her driveway, she then went to back out of her driveway and vehicle #2 struck her. Aghavni Gevorgyan was witness. Aghavni confirms Azad's version of events. Aghavni and Azad are known to each other. Azad was transported to NWH. I took pictures, no traffic officers working.

ON 4-26-19 AT APPROX. 1514HRS. WHILE WORKING N492 I TOOK A REPORT FOR CITY VEHICLE MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT 311 WATERTOWN ST. I SPOKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES HE WAS TRAVELING W-BOUND ON WATERTOWN ST. WHEN HE WAS HIT IN THE RIGHT SIDE OF HIS VEHICLE BY VEHICLE #2. VEHICLE #2 STATES SHE WAS ENTERING THE TRAFFIC LANE FROM A PARKED POSITION IN FRONT OF 311 WATERTOWN ST. DRIVER STATES WHEN SHE ENTERED THE LANE SHE THOUGHT VEHICLE #1 HAD ALREADY PASSED AND WAS UNABLE TO AVOID HITTING HIS VEHICLE. VEHICLE #1 HAD SCRAPES ALONG THE RIGHT SIDE OF HIS VEHICLE. VEHICLE #2 HAD LEFT FRONT END DAMAGE. ALL PARTIES REPORTED NO INJURIES. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES. CAPT. DOWLING ( N600 ) TOOK PICTURES OF THE VEHICLE DAMAGE AND FORWARDED IT TO THE NEWTON I.T. BUREAU. CLEARED WITHOUT FURTHER INCIDENT.

On 04/27/2019, while assigned to N494, I, Officer Conary, responded to Commonwealth Ave at Lowell Ave for a MVA. Upon arrival, I observed two sedans in the middle of Commonwealth, facing opposite directions. I told both operators to pull to the side of the road. Operator of MV1 stated that he was traveling Southbound on Lowell Ave and took a left turn onto Commonwealth Ave going Eastbound. When he took the turn, he said he took it too sharp and got distracted with a lip balm in his car. He looked down because it fell on the side of the driver's side door. When he looked down he entered the Westbound lane of traffic and hit MV2. Operator of MV2 stated that she was traveling Westbound on Commonwealth Ave when she saw MV1 swerving into her lane. She attempted to avoid MV2 hitting her, but MV1 hit her head on. Both operators were offered and declined medical attention. Both vehicles were driven from scene. No further incident to report.

Operator 1 of MV 1 was traveling on Hammond St when MV 1 was struck on the drivers side by MV 2. No injuries to Operator 1 and moderate damage to MV1. Operator 2 of MV 2 was stopped at Hammondswood Rd and looked right, then left and did not look right and proceeded forward and did not see MV 1. Moderate damage to MV 2 and no injuries to Operator 2 or the passenger. Both vehicles were able to be driven from the scene.

ON 5-1-19 AT APPROX. 1811HRS. WHILE WORKING N498 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT THE INTERSECTION OF CHESTNUT AND WINTER ST. I SPOKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES SHE WAS STOPPED AT THE STOP SIGN AT ELLIS AND CHESTNUT AND WAS PREPARING TO CROSS OVER CHESTNUT ONTO WINTER ST. DRIVER STATES AS SHE ENTERED CHESTNUT VEHICLE #2 CAME OVER THE HILL TO HER LEFT ON CHESTNUT AND SHE WAS UNABLE TO AVOID HITTING HIM. VEHICLE #2 STATES HE WAS TRAVELING S-BOUND ON CHESTNUT. AS HE CAME OVER THE HILL ON CHESTNUT ST. HE STATES VEHICLE #1 PULLED ONTO CHESTNUT AND HE WAS UNABLE TO AVOID HITTING HER. VEHICLE #1 HAD LEFT FRONT END DAMAGE. VEHICLE #2 HAD RIGHT FRONT END DAMAGE. BOTH VEHICLES WERE OPERATIONAL AND ABLE TO LEAVE THE SCENE ON THEIR OWN. ALL PARTIES INVOLVED REPORTED NO INJURIES. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES. CLEARED THE SCENE WITHOUT FURTHER INCIDENT.

Ashley Tartarilla was operating Vehicle #2 W/B on Dedham St. Ashley states that she had a green light at the intersection of Brookline St. Ashley states that when she went through the intersection she was struck on the right side by vehicle #1. Maxine Sheff was operating vehicle #1 on Brookline St attempting to merge onto Dedham St. Maxine states that she looked to the left and didn't see any cars. Maxine then pulled out she struck vehicle #2. No injuries, no tows.

Bonney Mastaby was driving vehicle #2 S/B on LaGrange St. Bonney states that when she got to the Vine St intersection vehicle #1 cut in front of her trying to take a left turn. Vehicle #1 then struck Bonney vehicle. Francisco Paz-Sorto was operating vehicle #1 N/B on LaGrange St. and states that he was trying to turn on to Vine St and crashed with vehicle #2. Francisco does not a have license and was issued citation #T1272112 Ch. 90 Sec. 10 unlicensed operation of a m/v. Vehicle #2 was towed by AAA. Franciscos wife got a ride to the scene and drove the vehicle #1 home.

Operator of MV1 states he was parked in a meter parking spot on Watertown Street when he put his left directional on to pull out into oncoming traffic. Operator of MV1 states MV2 stopped to let him pull out of the parking spot and when he did MV2 traveled with the flow of traffic and MV1 struck MV2's right front tire. MV1 sustained minor damage to the left front fender. Operator of MV2 states he was traveling westbound on Watertown Street, which was bumper to bumper traffic. Operator of MV2 states when the traffic light turned green he proceeded with the flow of traffic and that's when MV1 pulled out of the parking spot and struck MV2's right front tire. MV2 sustained minor damage to the right front tire rim.

MV1 was travelling WB on Beacon St when MV2 cut across his lane of travel causing him to strike the passenger side of MV2. MV2 was attempting to turn NB onto Walnut St from Beacon when he was struck by MV1. Operator 2 states he believed he had enough time to make it across the intersection and believes MV1 was speeding. Operator 1 states he was operating at the speed limit (30MPH) and MV2 cut in front of him. Both parties had the green light at the intersection. No injuries reported by any party. Parent of operator 2 signed a patient refusal based upon the operators age. Both vehicles were towed due to air bag deployment. MV1 sustained minor damage to the front end. MV2 sustained moderate damage to the passenger side.

On Sunday, May 5th, 2019 at approximately 1315 hours while working the City of Newton detail for the Walk for Hunger I was standing at my assigned post at the intersection of Centre Street and Willow Street. At the time, the road surface was dry and the weather was cloudy. Traffic was particulary heavy as there were several streets in the area still blocked off for the Walk for Hunger and there were participants still walking the route in the area. My attention was drawn to the intersection of Centre Street and Bowen Street as there was a motor vehicle travelling southbound on Centre Street with their left turn signal on attempting to take a left turn on to Gibbs Street. Gibbs Street was temporarily blocked to non-residential motorists as there were several pedestrians in the road crossing over Gibbs Street. As the motor vehicle attempted to turn, there were several pedestrians in the crosswalk crossing over Gibbs Street. The vehicle had stopped halfway into the northbound lane on Centre Street blocking on coming traffic. The operator appeared to realize Gibbs Street was blocked and attemped to turn the vehicle back into the southbound lane to continue southbound on Centre Street. As the vehicle turned back into the southbound traffic lane, the vehicles rear end collided with Officer Richard Clements, who was operating NPD motorcycle unit 533. Officer Clements was traveling southbound on Centre Street and was passing the vehicles in the southbound traffic lane which were currently all at a standstill. Officer Clements had his emergency lights activated as he was on his way to remove traffic cones from the street to open up all the traffic lanes along the event route. Officer Clements was working the City of Newton detail for the Walk for Hunger assigned to the motorcycle to assist with traffic. Officer Clements front wheel, left side of the motorcycle and left leg collided with the rear end of the motor vehicle. The impact of the crash caused him to fall off the left side of the motorcycle and land on the street face first. At this time I immediately ran from my post to Officer Clements location and radioed dispatch that a motorcycle officer was down and that I needed assistance. As I approached Officer Clements he was face down, unconcious and breathing. I was able to roll Officer Clements into the recovery position and at this time Officer Clements regained conciousness and was able to speak and stated " What happened? What was that lady doing she turned right into me". I advised Officer Clements that he had just been involved in an accident and to stay still and that help was on the way. The operator of the motor vehicle identified as Helen Leibner (S52717988) stopped her vehicle immediatley after the crash and got out of her vehicle to offer her assistance. I advised Helen and several other witnesses and bystanders who were now crowding around Officer Clements and I to back away from the scene and to not move any of the vehicles involved. Officer Selig arrived on scene and was able to help crowd control until further help arrived. Helen and all witnesses were advised to stand by for police to take their statements. While tending to Officer Clements he was disoriented and made several attempts to move his body to which I had to physically hold him still to prevent possible further injuries. While speaking with Officer Clements I noticed blood coming from the inside of his mouth. Officer Clements was in a noticeable amount of pain. He complained of head, back and left leg pain. Officer Clements was wearing a yellow ANSI certified Newton Police reflective jacket and a Newton Police issued motorcycle helmet. Once the paramedics and fire personnel arrived I was able to speak to the operator of the motor vehicle and several other witnesses. Helen stated she was traveling southbound on Centre Street operating her blue 2014 Audi A4 (MA REG 213BC9). Helen states she was attempting to take a left turn onto Gibbs Street and activated her left turn signal. Helen states she had stopped halfway into the northbound lane as several pedestrians were crossing Gibbs Street in the crosswalk at the time. Helen states after the pedestrians cleared she saw the traffic cones blocking the street and realized the street was closed. Helen states she turned her vehicle back into the southbound traffic lane and did not see Officer Clements or the motorcycle until impact. Helen was not injured and her motor vehicle sustained moderate rear end damage. Her motor vehicle was able to drive away from the scene. Witness Adron Gordon (S90357161) stated he was participating in the Walk for Hunger and had just crossed over Gibbs Street when the crash occured. He stated he witnessed the Officer on the motorcycle with the emergency lights on traveling southbound on Centre Street behind the motor vehicle. The motor vehicle stopped in traffic attempting to turn left onto Gibbs Street. He states then the vehicle made a slight turn back into the southbound traffic lane and the motorcycle collided with the rear end of the motor vehicle. He states the Officer then fell off the motorcycle landing in the middle of Centre Street. Witness Elizabeth McCarthy (S00242346) and Katherine Regan (S43486601) stated they both were in the same vehicle together stopped in traffic in the northbound lane of Centre Street just before the set of lights at Centre Street and Bowen Street. Both witnesses stated they do not remember if the driver of the motor vehicle had the turn signal on but they did see her attempt to turn onto Gibbs Street. They stated they also saw the motorcycle with emergency lights activated travel southbound on Centre Street approaching behind the vehicle. Witnesses state the motor vehicle then suddenly turned back into the southbound lane and the motorcycle collided with the passenger side tailight of the motor vehicle. Both witnesses state the motorcycle attemped to turn away from the vehicle to avoid the crash but was unable to do so. Witness Nina Wang (S39460985) stated she was parked on the southbound lane of Centre Street just prior to the crash scene. Nina states she was in her City of Newton DPW marked vehicle with the hazards on as she was working to assist with traffic safety for the Walk for Hunger. Nina states she witnessed the motorcycle travel southbound on Centre Street and pass the vehicles stopped in the traffic lane. Nina states the motorcycle passed the traffic using the left side and had the emergency lights activated. Nina states the motorcycle was just passing through the green light at Centre Street and Bowen Street when the motor vehicle that was stopped at the lights attempting to take a left turn onto Gibbs Street. Nina states the vehicle then abruptly jerked her car to the right and collided with the motorcycles left side. Nina states she heard a loud crash and witnessed the Officer fall off the motorcycle and land face first onto the street. The motorcycle's final resting place was on its left side approximately two feet away from where Officer Clements was located. The motorcycle sustained moderate damage to its left side, including the front wheel well and undercarriage. The motorcycle left scuff marks on the street which were measured and photographed. Several photographs of the scene and damage to both vehicles were taken and submitted to the IT Bureau. The motorcycle was towed by Todys. Centre Street and Bowen Street are both public ways maintained by the City of Newton. The injured motorcyclist, Officer Clements, was transported to Saint Elizabeth Hospital with fire fighters on board. I was able to follow up with Officer Clements on the phone the next day who was admitted over night at the hospital. Officer Clements stated he has no recollection of the accident and can only remember driving down Centre Street to begin to pick up cones to open up all lanes of traffic. Officer Clements stated he suffered a brain bleed, head, back, left foot and right thumb injuries. A Massachusetts Uniform Citation was issued and mailed to Helen for Chapter 89 Section 7A, failure to yield to emergency vehicle, police motorcycle.

MV1 stated he was attempting to turn left onto Crafts St from Walnut St when the back end of his tractor trailer made contact with a light pole and knocked it over due to "the lane not being made for these kinds of trucks." The trailer sustained minor damage and the light pole was completely uprooted and across the sidewalk. Photos of the scene were captured and turned over to the IT Bureau. Daigle Electric notified of the damage to the pole.

On Tuesday May 7th, 2019 at approximately 13:15 hours I responded to the area of 30 Langley Road for an auto vs. pedestrian accident. At the time of the accident the weather was sunny and the road conditions were dry. Langley Road is a public way maintained by the City of Newton. Upon arrival I first spoke to the pedestrian who was inside the ambulance getting medical attention. The pedestrian identified as Donna Kalikow (S24151638) stated she was standing in an open parking space for a few seconds, attempting to walk across the street to enter J.P. Licks. Donna stated she was standing in the marked parking space waiting for traffic to stop to safely cross the street. Donna states the cars stopped to let her cross and she proceeded into the roadway. She states MV1 was attempting to back into the parking space that she was standing in and struck her right side and knocked her to the ground. Donna states the operator of MV1 stopped the vehicle when he realized he had struck her and got out to contact 911. Donna appeared to have minor injuries and complained of head pain. Medics transported Donna to NWH to be further evaluated. The operator of MV1 identified as Christopher Moos (S24090966) stated he was attempting to parallel park into an empty parking space and never saw the pedestrian standing in the space. Christopher stated as he was backing up into the space he was notified by a bystander that he just struck a pedestrian. Christopher stated he immediately stopped the vehicle and got out to assist the pedestrian and contact 911. Christopher stated he was not travelling no more than 5mph at the time of the accident. MV1 did not have any damage to the vehicle and was able to leave the scene. Christopher reported no injuries. Several photographs were taken of the scene and submitted to the IT Bureau. I mailed the operator a Massachussetts Uniform Citation for a City of Newton Ordinance violation for Section 19-75 Failure to use care when backing.
