Operator of vehicle one stated that while driving east bound on Crafts St at Albemarle Rd vehicle two darted out into the path of his vehicle from the south bound lane of Albemarle Rd. Operator of vehicle one stated that he attempted to avoid a crash with vehicle two by applying his vehicle's brakes and swerving to his left. Operator of vehicle one stated that his vehicle's front bumbler made contact with vehicle two's front driver's side fender and bumper. Operator of vehicle one stated that he was not injured his vehicle was not towed. Operator of vehicle two stated that he had stopped at the stop sign in the north bound lane of Albemarle Rd at Crafts St. Operator of vehicle two stated that he saw vehicle one driving east bound on Crafts St and he pulled out into the east bound lane of Crafts St in font of vehicle one believing that he had enough time to cross over Crafts St . Operator of vehicle two stated that vehicle one's front bumper struck his vehicle's front driver's side. Operator of vehicle two stated that he was not injured and his vehicle was not towed. Passenger in vehicle two Zbigniew Saletnik stated that he was not injured and had little recollection of the crash. All Streets are public ways in the city of Newton. I issued operator of vehicle two MA Citation T0340520 and cited him for MLG 89/8 , failure to yield. I took photos of the vehicles and crash area, the disk was downloaded by the NPD's IT bureau.

VEHICLE #2 WAS DESCRIBED AS A RED KIA SUV, MA REG 36TX70. THE OPERATOR APPEARED TO BE A MALE WITH DARK HAIR. A REGISTRATION QUERY SHOWED THE OWNER TO BE A BRIANNA COSKIE, 27 N MAIN ST UPTON MA. UPTON POLICE LATER MADE CONTACT WITH THE OWNERS SISTER WHO RELAYED THE MESSAGE TO BRIANNA TO CONTACT NEWTON POLICE. A SHORT TIME LATER I RECEIVED A CALL FROM BRIANNA COSKIE SHE REPORTED THAT HER BOYFRIEND WAS OPERATING HER VEHICLE THIS MORNING. SHE HAD ALREADY CONTACTED HIM PRIOR TO CALLING POLICE. HE TOLD BRIANNA THAT SOMEONE CUT HIM OFF THIS MORNING BUT WASN'T INVOLVED IN ANY ACCIDENT. I TOLD BRIANNA WHERE THE DAMAGE WAS ACCORDING TO THE OTHER DRIVER/ PASSENGER SIDE FRONT QUARTER. Narrative:
MV1 (MA Reg: 51JD62, 2007 green Toyota Pruis) operated by Marcia Gittes (DOB 11/29/1948) was traveling Southbound on Walnut St when it collided with a pedestrian in the crosswalk at Walnut St/Foster St. MV1 collided with a man, later identified as Jerome Shectman (DOB 10/7/1964). He had an injury to the back of his head, right hand, left shoulder and lower spine and was transported to Beth Israel Hospital. Witness, Patrick Clare-O'Brien saw the crash take place. He stated that the pedestrian, Jerome was crossing in the crosswalk traveling Westbound towards Foster St, successfully passed through the lane where vehicles were traveling Northbound on Walnut St and was then hit by the Toyota Prudish in the lane going Southbound. Jerome was already halfway through the crosswalk at this point. It was dark, but there was a streetlight directly over this crosswalk on Walnut St creating a semi-lit area for visibility. Mrs. Gittes stated that she didn't see him because he was wearing dark clothing and it was dark outside. She saw him when it was too late. There was no damage to her vehicle. Mrs. Gittes was given a monetary citation (T0646789) for 89/11 Crosswalk Violation. pictures were taken at the scene and the photo SD card was sent to IT. Traffic Bureau update (Officer Gaudet): I was asked to follow up with the injured pedestrian involved in this accident, Jerome Schectman (S77606276). Schectman stated he was attempting to cross Walnut Street east to west towards Foster Street. Schectman stated he entered the crosswalk after observing no cars traveling towards him in the Northbound lane of Walnut Street. Schectman stated he began walking across the street and stopped in the middle of Walnut Street after observing 2 vehicles traveling towards him in the Southbound lane. Schectman stated as the first vehicle went by, he stared at it as it traveled past him and down the street. Schectman stated as he looked back at the second car it was traveling right at him and he was unable to get out of the way. Schectman stated the vehicle hit him on his right side. Schectman listed his injuries as follows: fractured vertebrae, laceration to the back of his head, neck pain, and left shoulder/arm pain.

On Sunday January 6, 2019 at approximately 1747hrs, I responded to the intersection of Pleasant Street and Centre Street for a report of a city motor vehicle crash. Upon arrival I located all vehicles pulled off on Pleasant Street about 50 feet from the Centre Street intersection. The City vehicle involved was Newton Police Cruiser N 496 ("MV #2") and it was driven by Officer S. Siegal. Siegal reported that he was directly behind MV #1 and observed the vehicle make a left turn onto Pleasant Street which was against the one way sign. Officer Siegal reports upon seeing this illegal turn he immediately turned his emergency lights and was trying to make a motor vehicle stop. The vehicle immediately stopped and while trying to back up and turn around, the vehicle backed into his cruiser. Officer Siegal reported that the vehicle was backing at just a few miles per hour. MV #1 was MA Reg. 3ZN523 a 2004 Nissan Sentra. This vehicle was operated by Huihui Tang. Tang reported that she was trying to go to CVS at that intersection when she turned onto Pleasant Street. Tang reported when the emergency lights were activated she tried to turn around and made minor contact with the police cruiser. During our conversation there was a bit of a language barrier and Tang appeared confused on why we needed to do a report. There was no injuries or any damage to any vehicle. Centre Street and Pleasant Street are Public ways in the City of Newton. Officer Siegal issued Tang MA Uniform Citation T0646880 for violating 19/85 for operating the wrong way. This citation was mailed to Tang. The initial citation issued on scene was later voided due to an address error. Officer Siegal also took pictures of both vehicles. Tang was advised on the process of what to do following a crash and reported she understood.

Narrative:
On the above date and time I responded to the area of Watertown Street at Pearl St for a report of a traffic signal that was knocked down. Upon arrival on scene I observed the traffic signal knocked over and tire impressions of a possible sidewalk plow. I took pictures of the crash site area and will have them downloaded at the I.T. Bureau. I canvassed the area and found a City of Newton sidewalk plow with the operator. I spoke to the operator who stated he observed the damaged traffic signal and moved it out of the way but he did not have contact with it. I had dispatch notify the traffic signal company due the lights in the intersection being out. I also canvassed the area for cameras with negative results.

The operator of MV1 stated he was traveling Northbound on Centre St. at the intersection of Hyde Ave. when a vehicle in front of him braked. When he stepped on his brakes to slow down, MV1 skidded on ice off to the right into the "Speed Limit 25" sign, knocking it over. Tody's responded and pulled the vehicle off of the sign and the vehicle was able to be driven from the scene safetly. No injuries were reported. MV1 was stopped at a red light north on Tremont St at Waverly Ave. MV2 was stopped behind MV1. MV2 reached for something and her foot slipped off the gas and she bumped the rear of MV1. All parties agreed that it was a tap from behind and that there was no damage to either MV. However, there were 2 special needs students on the van that go to NNHS. At approximately 1615HRs. on Thursday, January 24, 2019, I was dispatched to the intersection of Elliot St. and Boylston St. for a report of an MVA. Upon arrival, I located the operator of MV1 (MA Pass: 1NB459) who had pulled into the BP gas station. He stated that he was traveling Northbound on Elliot St. when his traffic signal turned red and he brought his vehicle to a complete stop at the intersection. He stated that at this time, he was rear ended by MV2. He and the operator of MV2 then got out of their respective vehicles and spoke about the collision, after a few moments, the operator of MV2 got in her vehicle and left. Using the picture of MV2's license plate (MA Pass: 3EY477) which he provided me, I was able to contact the operator of MV2 at her residence. She stated that she was traveling Northbound on Elliot St. and observed MV1 come to a stop and she applied her brakes and came to a complete stop aswell, without making any contact to MV1. She further stated that the operator of MV1 became agitated and started to yell at her. She then left because she asserts that there was no collision and she was concerned by the operator of MV1's behavior. I observed no damage to MV1's rear bumper area, with the exception of an old dent, approx. the size of a quarter, which was highlighted with some type of permanent marker. I observed no damage to MV2's front bumper area and I noted large, pronounced license plate bolts affixing MV2's license plate to its bumper. The operator of MV1 stated that he was suffering from back pain due to the collision and elected to drive himself to Carney Hospital. Due to there being no damage to either vehicle and the large plate bolts on MV2, which would have likely caused notable damage to MV1's bumper, I will not be charging the operator of MV2 with leaving the scene of an accident at this time. Both parties have been advised to contact their insurance companies and I cleared without incident.

On Tuesday, January 29, 2019, while assigned to Traffic unit N525, I responded to the intersection of Cherry Street and Webster Street, Newton for a report of a motor vehicle accident involving a City of Newton owned vehicle. Cherry Street and Webster Street are both public ways maintained by the City of Newton. The weather at the time of the accident was overcast. The road surface was dry. The operator of MV1, John Bryant III (MA: S61611545), stated he was operating a City of Newton owned Volvo BL70 (MA MVN: M84236) Eastbound on Webster Street past Cherry Street. Bryant stated as he came to a stop in traffic, MV2 crashed into the rear of the vehicle he was operating. I observed minor damage to the rear bucket area of MV1. Bryant reported no injuries on scene and signed a patient refusal with Newton Medics. Bryant works for the Utilities Department with the DPW. The operator of MV2, Andrew Bell (NJ: B23900426706004), stated he was operating his 2013 Volkswagon (NJ: C54KKJ) Eastbound on Webster Street past Cherry Street. Bell stated he was following MV1 a little too close while they were traveling down Webster Street. Bell stated while traveling down Webster Street he looked down quickly but does not remember why, and when he looked up MV1 had come to a stop. Bell stated he could not avoid crashing into the rear of MV1. I observed significant damage to the front hood/windshield area of MV1. Bell was transported to Newton Wellesley Hospital with a minor injury. Bell was provided with Massachusetts Uniform Citation T1272991 in hand for Newton City Ordinance Chapter 19 Section 71 (Following Too Closely) . Pictures were taken of both vehicles and submitted to the IT Bureau.

MV1 stated he was "driving to fast around the corner and hit the pole." MV1 actually hit a curb, took down a street light, went up on the island hitting another street light prior to hitting the curb. The second light he struck fell on MV2. MV1 sustained heavy front end damage and was removed from the scene by Tody's Towing. MV2 stated that MV1 "came flying across the intersection, crashed into 2 poles, one of which hit my car and he crashed over there." I then observed minor damage to MV2, however it did not require a tow. Both parties signed patient refusals with Cataldo Ambulance. Operator of MV1 was arrested and charged with the following: M.G.L. 90/24/J OUI Liquor M.G.L. 90//24/E Negligent Operation M.G.L. 89/4A Marked Lanes Photos of the scene were captured and turned over to the IT Bureau. Daigle Electric notified of the damaged lights.

The OP of MV#1 states while travelling northbound on Moulton Street, he observed MV#2 sanding the intersection of Cornell Street so he stopped. MV#2 then started reversing in his direction and when MV#2 didn't stop, the OP. of MV#1 then honked his horn but they still collided. The OP. of MV#2 states he was sanding Cornell Street and was doing a U-turn at the intersection of Moulton Street. He didn't observe MV#1 being on Moulton Street at this time so continued sanding the area. When he reversed, that's when his rear tire collided with MV#1. Pictures were taken by me.

On 2/13/19 at approximately 2045 hours, I responded to a single vehicle accident. The operator was driving northbound on Woodland rd and was taking a left hand turn on to Maple st when the car slipped on the snow/ice and collided with the no parking sign on the sidewalk. No injuries reported. Todys was notified to help assist getting the vehicle off of the sign. Two pictures were taken and submitted. The city was notified of the sign being down and in need of a replacement.

Narrative:
At approximately 0110HRs on Sunday February 17, 2019, I was dispatched to the area of Dedham St. and Spiers Rd. (both public ways in the City of Newton), for a report of a motor vehicle vs pole accident with injury, airbag deployment and entrapment. Upon arrival, I exited my cruiser with my medical bag and AED and went to assist the motorist involved in the crash. MV1 (MA Passenger: 255ZA9) was completely off the roadway and on the shoulder of the road (facing Eastbound) with its nose contacting the light pole ( Verizon Pole #:103/80), which was now broken and listing to the side. Attempts to open the doors were unsuccessful and I attempted to communicate with the operator through a partially broken driver's window frame which was bent. The operator was slumped forward with his hand cradling his head and he was unable to speak, but was breathing. Newton Fire arrived and attempted to gain access to the vehicle via several manual tools with no success and instead deployed the Jaws of Life which was successful. Newton Fire also made entry into the rear passenger compartment because we viewed a child's car seat and checked with negative results for any child in the vehicle. Once the operator was cut out of his seatbelt and safely out of MV1 and on board Newton Medic 1, I entered the ambulance where he was identified as Mark Topham. It should be noted that Topham was having difficulty answering basic questions such as his name, date of birth, today's date, Etc. At one point when he was asked what today's date was, he gave his birth date of February 21, 1983 and it took him several attempts to say his last name, I had to run him over the air with a partial name to ascertain who he is. Also of note, in my presence and that of Sgt. Devine, Topham informed the medics that he had been consuming alcoholic beverages prior to the crash. Upon viewing the scene, I noted that the roadway was lit and I did not observe any skid marks on the dry, ice-free payment, indicating that there was unlikely any braking or swerving to avoid the collision. I spoke to the reporting party, Lindsay Giardina, she stated that she was traveling on Dedham St. when she heard what she believed to be a motor vehicle crash. When she came upon said crash, she attempted to assist the operator, as she was an off duty firefighter. She was unable to gain access to the inside of the vehicle, but communicated to the operator in the same manor as I did. She stated that she was able to smell the odor of an alcoholic beverage emanating from the operator. Topham was transported to Beth Israel of Boston, where he was assigned patient record number 3109921. I attempted to speak with him at the hospital, but was unable to do so because of his current state; I noted several injuries on Topham, such as a bloody lip, a wound to his lower left side and a wound to his left leg. Due to Topham's statements onboard of Medic 1 and Lindsay Giardina's observations, I will be charging Topham with one count of violating MGL Ch.90 S. 24 K OUI Liquor 2nd Offence, one count of violating MGL Ch.90 S. 24 E Negligent Operation of a Motor Vehicle and one count of violating MGL Ch. 89 S. 4A Marked Lanes Violation via MA Criminal Citations which I placed in the mail. Additionally, I will be seeking a subpoena for Topham's blood work from the hospital. Note: The crash occurred on the Newton/Boston line, Newton dispatch's GPS indicated that the crash was in Newton. I took several pictures of the scene and have forwarded them to IT to be attached to this report. I completed and filled an NPD Motor Vehicle Tow Inventory Form and Officer Boudrot stood by for the tow.

The operator of Vehicle #1 stated that he stopped, prior to proceeding through the Stop Sign Southbound at Lewis St, and Newtonville Ave prior to colliding with the bicyclist. The bicyclist stated that he was travelling Westbound on Newtonville Ave prior to colliding with Vehicle #1. Vehicle #1 was cited for a violation of M.G.L. Ch.89/S.9-Stop Sign. It should be noted that the bicyclist was wearing a helmet, and a rear flasher, however wore no other bright, or reflective clothing. All parties declined medical attention. Pictures of the scene were taken by me, and placed in the IT box.

Operator of MV1 states she was driving Northbound on Centre St. She states she was in the right (straight ) lane, going the speed limit. She states MV2 attempted to turn left in front of her, and she was unable to avoid them. She stated MV2 struck her vehicle on the left front side, sending her off the road and into the traffic signal on the northeast side of the roadway. Operator of MV2 states he was driving southbound on Centre St, attempting to turn left onto Walnut St. He stated he did not see MV1 until the least second, because of the cars in the opposite turn lane. He stated he attempted to stop, but believed MV1 was speeding and he did not have time. Witness 1 was directly behind MV2. He stated it looked like MV2 was attempting to turn left in front of him. He stated MV1 did not appear to be speeding, and that MV2 turned into MV1. He stated he did not think the two cars could see each other until the last second. Witness 2 was in the opposite turn lane. She also stated it appeared MV1 was driving the proper speed, and MV2 turned into MV1. MV1 was towed by Tody's. MV2 was moved off to the side on Walnut St to wait for AAA. 2 parties from MV2 were transported to NWH with minor injuries. Operator of MV2 was given in hand MA citation# T1441667 for 89/8, Fail to yield while turning left. A traffic signal was taken down, and Dagle electric was notified to respond. All other lights were functioning properly. 4 photos were taken of the damage.

On Monday, March 11, 2019 while assigned to Traffic unit N525, I responded to the area of Watertown Street and Adams Street, Newton for a report of a motor vehicle/pedestrian accident. The accident was reported to Newton Police Dispatch by Newton Police Officer Charles Guarino. Officer Guarino identified the pedestrian involved in the accident as Newton Police Officer James Selig. Watertown Street and Adams Street are both public ways maintained by the City of Newton. After completing my investigation, the operator of MV1, Colleen Butaney (S20012566) was issued Massachusetts Uniform Citation T1272995 for Chapter 90, Section 25 (Fail to Stop for a Police Officer) and Chapter 89, Section 4A (Marked Lanes Violation). Butaney was advised and the citation was placed in her property in Locker #4 in the NPD holding area prior to her release. Officer T. McCarthy completed and submitted a criminal application for Butaney for Chapter 265, Section 15b (Assault with a Dangerous Weapon, MV). Please refer to incident report #19009204 for statements from all parties involved. Photos were taken of the area/vehicle and submitted to the IT Bureau. The vehicle involved, (MA: 531PX6) was removed from the roadway by Tody's towing. Officer Marini completed a towed motor vehicle report form. Surveillance footage of the incident was recovered from a local business and was submitted to the IT Bureau/Support Services. A property and evidence form was completed and submitted to Officer Manouk.

Narrative:
On Friday March 15th 2019 at approximately 0730 hours I responded to the intersection of Beacon Street and Herrick Road for a report of a hit and run involving a bicyclist. The weather at the time of the accident was cloudy and the road conditions were dry. Both streets are public ways in the City of Newton. Upon arrival I observed the victim identified as Glenn Butcher (MA OLN S36552335), inside the ambulance getting medical attention. Butcher was getting a bandage applied to his left leg and complained of leg, back and neck pain. Butcher appeared visibly shaken and in pain. Butcher stated he had the green traffic light and was travelling eastbound on Beacon Street crossing Centre Street and then continuing straight crossing Herrick Road. Butcher stated MV1 was travelling westbound on Beacon Street and took a left turn onto Herrick Road and collided into his bicycle. Butcher states upon impact he flipped over his bicycle and landed on the hood of MV1, before rolling off and landing on the ground. Butcher states he was wearing his helmet which was present. Butcher states his bicycle ended up partially under MV1. Butcher states the operator of MV1 pulled his vehicle to the side of the road and got out of his vehicle and spoke to several witnesses briefly. Butcher states the operator then left the scene without properly exchanging information and did not wait for the police to arrive. Butcher described the vehicle as a dark gray Honda, and described the operator as an older white male, 5'10 approximately 80 years old with white hair. Butcher was also able to provide an email address for another bicyclist who witnessed the accident. The witness left his information with Butcher prior to my arrival. Fire fighters on scene were able to get the registration number of the vehicle from the witness (MA REG 4CX148) and gave it to Officers on scene. Butcher was transported by Medic 1 to NWH with minor injuries. His bicycle had visible damage to the frame area and skid marks were present on both tires. 19 photographs of the scene and the bicycle were taken and sent to the IT dept. email. Officer Gabriel transported the bicycle to the station where it was logged into Property & Evidence for safekeeping. Dispatch was able to query the registration which came back to a gray, 2010 Honda Accord registered to a Matthew Sugarman (MA OLN S38601397), out of W. Roxbury. Dispatch contacted the Boston Police Dept. and had them send an Officer to the address to try to make contact with Sugarman. Sugarman was advised to contact the Newton Police Dept. and I was able to speak with him on the telephone. Sugarman states he was driving the vehicle today and was involved in an accident with a bicycle. Sugarman states he was travelling westbound on Beacon Street and was taking a left onto Centre Street not Herrick Road. While speaking with Sugarman he seemed confused as to his location during the accident and was adamant that he was turning left onto Centre Street and not Herrick Road. I advised Sugarman that I was on scene with the first responding Officers and the accident clearly occurred at the intersection of Beacon Street and Herrick Road based upon the final resting place of the bicycle. Sugarman went on to state that his vehicle was already turning when the bicycle struck his vehicle and he believed it was the bicyclists fault. I advised Sugarman it appears the bicyclist had the right of way since it was traveling straight and that because his vehicle was taking a left turn, he failed to yield for on coming traffic at an intersection. When asked why he did not stay at the scene of the accident Sugarman stated he got out of his vehicle and spoke to the bicyclist and he "seemed okay". Sugarman states he then left the scene while the victim was still sitting on the ground. I asked Sugarman if he had ever been involved in accident before and he replied "no." I advised Sugarman that I am currently looking at his drivers history and see nine accidents that he was involved in. Sugarman stated "he has never been in accident like this before." I then asked Sugarman what the law states when you are involved in an accident and he was not able to provide me with an answer. I advised him that the law states you must stop and exchange information before leaving the scene of an accident. Again he stated the bicyclist seemed okay and "he had somewhere to be so he left." Sugarman has an extensive drivers history including nine surchargeable accidents, reckless operation, two different charges of leaving the scene property damage and two immediate threat affidavits filed against him. I advised Sugarman that I will be mailing him a citation for Ch89 s8 Fail to yield to oncoming traffic, and Ch90 s24 Leaving the scene of personal injury. I also filed an immediate threat form with the registry. I emailed the witness using the information he provided to the Newton Fire Dept that was on scene and received a phone call back from the witness identified as Bruce Adams. See witness report for his statements. I contacted the Bank of America located at 808 Beacon Street in Newton, as they are the closest business to that intersection with video surveillance. I spoke with George Walsh (781-834-8275) who stated they do not have any working cameras on that side of the building to the bank. For this report the bicyclist is entered as a non-motorist for validation purposes. Witness Bruce Adams (MA OLN S96398108) stated that he was riding his bicycle travelling behind the victim about 100 feet away. Adams states he was watching the victim and had a clear view of him as he travelled behind him, through the green traffic light heading eastbound on Beacon Street. Adams stated he witnessed MV1 travelling westbound on Beacon Street take a sharp left turn onto Herrick Road which turns into Union Street. Adams stated that the vehicle turned left colliding into the bicycle and there was no possible way the bicyclist had time to stop to avoid the collision. Adams stated the victim flew over his handle bars and fell on top of MV1's hood and then landed on the ground. Adams stated the victim's bicycle was stuck under MV1's front bumper. Adams stated the operator of MV1 backed his vehicle up with the bicycle still attached to his vehicle bumper and pulled MV1 over to the side of the road. The operator then got out of his vehicle and walked up to the victim who was laying on the ground visibly in pain. Adams stated the operator asked the victim if he was okay to which the victim did not immediatley reply as he was visibly shaken up. Adams stated the operator then walked back to his vehicle and attempted to leave the scene and that is when Adams stood infront of MV1 to stop him from leaving the scene. Adams stated he then removed the bicycle from underneath the bumper of MV1 and placed it on the side of the road. Adams stated the operator got out of his vehicle again and walked up to the victim and asked him "what do you want to do?". Adams stated the victim did not respond and that is when the operator got back into his vehicle and drove away, leaving the scene before any emergency response arrived. Adams stated he was able to memorize MV1's registration and stayed with the victim until police, fire and medics arrived. He stated he gave the fire dept the registration and his contact information prior to leaving the scene.

Operator of vehicle one ( MA reg 3986BV 2012 MERZ ) , Vladimir Krapchev stated that on 03/21/2018 at 08:56 hours he was stopped at a red light on Center St at Commonwealth Ave. Krapchev stated that his vehicle was heading south on Center St and was the third vehicle in line to turn west bound on Commonwealth Ave. Center St and Commonwealth Ave are both public ways in the city of Newton. Krapchev stated that " a red/maroon full size sedan fire department car " struck the front driver's side fender of his vehicle. Krapchev stated that the fire department car was travailing south bound on Center St in the north bound lane, passing stopped south bound Center St traffic. Krapchev stated that there was also north bound traffic on Center St that was turning from the west bound lane of Commonwealth Ave. Krapchev stated that the fire department car turned onto the east bound lane of Commonwealth Ave and did not stop. Operator of vehicle two Newton Fire Department LT Phillip McCully stated that he was driving Newton Fire Department Car 14 ( MA reg MVN MF461 2004 Ford Crown Victoria ) south bound on Center St with his vehicle's emergency lights and siren activated. Lt McCully stated that he was responding to a motor vehicle crash with possible entrapment on Hammond Pond Parkway. Lt McCully stated that he was passing stopped Center St south bound traffic in the north bound lane of Center St. Lt McCully also stated that there was north bound traffic on Center St and " it was tight between the cars ". Lt McCully stated that he was unaware that a crash had occurred. Lt. McCully stated that he did not fell and contact between his vehicle and Krapchev's vehicle. Lt McCully also stated that he did not hear the crash because of his vehicle's siren was activated. Krapchev's vehicle had minor damage and red paint marks on it's front driver's side fender. The Fire Department car had scuff marks on it's passenger side rear door. Neither vehicle was towed and both operators stated that they were not injured in the crash. I took photos of both vehicles and forwarded the disk to the NPD It bureau for down loading.

MV1, a City of Newton contracted school van, was the first car stopped at the red light north on Walnut St at Homer St. MV2 was slowing as she approached the red light and rear ended MV1. According to MV1, driver of MV2 got out of her MV and looked at both MVs. She then got back into her car and when the light turned green they both pulled over just past the intersection. MV2 asked MV1 if she hit him and he stated yes. She stated that she did not see his van, which is bright blue. MV2 operator stated to me that she was stopped at the traffic light and thought MV1 backed into her. When I told her MV1 statement she didn't argue. There was minimal damage to MV1. The outline of MV2 license plate was imprinted in dust on the bumper. There was no visible damage to MV2. I took pictures of the damage to MV1 and MV2. The following parents responded to sign refusals for their children: Wenlei Zhou for Sohia Zhou Katie Thatcher for Tristan Thatcher Both students went to school. The students go to the Newton Early Childhood school on Jackson Rd.

On Friday, March 29th 2019, at approximately 5pm, I, Officer Brooks, responded to the area of 450 Crafts street to speak with an operator who was involved in an accident. On my arrival I spoke with the operator of MV1(MA REG 28Z390), who stated approximately a half hour ago she was involved in a MVA with MV2(MA REG 7HA817), at the intersection of Crafts street and Albemarle road. The operator of MV1 stated she had been traveling westbound on Crafts street, when MV2 pulled off of Albemarle road heading southbound and struck her vehicle. I observed damage to the passenger side front quarter panel and wheel well of MV1. The operator stated she exchanged information with the operator of MV2, and they both went their seperate ways. MV1 is a rental car, and then the operator of MV1 called the rental car company they requested she make a police report. I was able to speak with the operator of MV2 via telephone. He states he was traveling on Albermarle road, and stopped at the stop sign at the intersection with Crafts street. After stopping he began to pull out, and his vehicle "scraped" MV1. The operator of MV2 stated he never saw MV1 coming. I saw pictures of the damage to his vehicle, which consisted of scratches on the drivers side front bumper and quarter panels. No injuries were reported and both vehicles were driven from the scene. I provided both operators with the report number.

On 04/01/2019, at 1736 hrs, I responded to the intersection of Washington St and Waverley Ave for a single vehicle MVA. Upon arrival, I observed a city sign with street names and a stop sign bent over to the ground. There were tire marks across the small island leading up to the signs at the intersection. The vehicle on scene had damage to its front passenger side. The operator of the vehicle stated he was traveling westbound on Washington St and attempted to turn left onto Waverley Ave when he ran over the island and struck the sign. He stated a vehicle was traveling eastbound on Washington St which sped up and caused him to crash. Pictures were taken of the city property damage and submitted to IT. The city was also notified to replace the signs.

MV1 a City of Newton school bus was stopped southbound on Parker St at Rte 9 east to turn left onto the ramp. MV2 began passing the bus on the right when the bus turned left onto the ramp. The right rear corner of the bus struck the front left wheel well of MV2. I took pictures of the damage to MV2 and TOT IT. I spoke with Regina Moody of transportation and she put me in contact with the driver of a bus who was in the area at the time of the MVA. On Thursday I met with Joseph Montuna who was in the area at the time of the MVA. His bus is 362. The side of his bus says Eastern Bus Company. It does not say Newton Public Schools anywhere visible. I looked at the right rear of the bus that would have come into contact with the MV. There was not even a smudge on the bumper. I am not positive that this is the bus involved. I have attempted to contact MV2 for clarification to positively ID the correct bus. The number provided goes directly to VM which is full. It is possible that this was a non Newton school bus involved that I am unable to identify.

The operator of vehicle 1 stated that with a green left turn arrow showing, and her left directional on she went to turn onto Waltham Street from Crafts Street when all of the sudden she hit a bicyclist. She stated she did not see the bicyclist at all prior to him hitting her car. The bicyclist stated does not remember anything from the accident at this time. The bicyclist was traveling southbound on Crafts Street and appears to have entered the intersection of Crafts and Waltham Street without stopping and while a red light was showing. He was wearing a helmet at the time of the crash, he was not wearing any reflective clothing(dark green jacket), and the bike did not have any lights. The bike was taken to support services for safe keeping by Ofc. J.March. Vehicle#1 was towed away by Tody's towing due to extensive damage. Bicyclist was transported to Beth Israel hospital for several trauma injuries. Pictures of scene was taken by me and sent to IT Bureau. Both witness's statements were the same. They stated that they were behind vehicle 1 in traffic and saw that she had on her left dirrectional signal and had a green left arrow signal when vehicle 1 went to turn left onto Waltham Street.

ON 4-8-19 AT APPROX. 0858HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE CRASH. UPON ARRIVAL AT THE INTERSECTION OF FOREST AVE. AND OTIS ST. I SPOKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES HE WAS TRAVELING S-BOUND ON FOREST HEADING TOWARD OTIS WHEN HIS VEHICLES RIGHT FRONT TIRE FELL INTO A LARGE HOLE IN THE STREET. UPON FURTHER INSPECTION I COULD SEE THAT A CATCH BASIN HAD COLLAPSED AND FORMED A LARGE HOLE IN THE STREET. THERE WERE NO VISIBLE SIGNS OF DAMAGE TO VEHICLE #1 AND THE OPERATOR REPORTED NO INJURIES. ADVISED TO CONTACT INSURANCE COMPANY. CITY OF NEWTON WAS NOTIFIED. I PLACED THREE ORANGE CONES AROUND THE HOLE TO AVOID FURTHER ACCIDENTS. COMP. LEFT THE AREA BEFORE PICTURES OF HIS VEHICLE COULD BE TAKEN.

On Wednesday, April 10, 2019 while assigned to Traffic unit N525, I responded to the intersection of Centre Street and Wesley Street, Newton for a report of a motor vehicle accident involving a bicycle. It was also reported that the parties involved were arguing with each other. The weather at the time of the accident was cloudy and the road surface was dry. Centre Street and Wesley Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Abdulaziz Stewart (S04075169). Stewart stated he was operating his 2017 Toyota Camry (MA: 669FM3) at the time of the accident. Stewart stated he came to a stop on Wesley Street (E) at Centre Street and was attempting to take a left turn onto Centre Street. Stewart stated there were no vehicles in the Southbound lane of Centre Street and there was a vehicle stopped in the Northbound lane of Centre Street motioning to him to take his left turn. Stewart stated as he began to start his left turn, a bicyclist crashed into the front passenger area of his vehicle. Stewart stated the bicycle had been traveling Northbound on Centre Street in the Southbound Lane. Stewart stated after the collision the bicyclist was yelling at him and called him a "Nigger". Stewart and his occupant, Miriam Stewart, reported no injuries. Miriam is a student at Bigelow Middle School. I observed minor damage to the front passenger quarter panel area of MV1. I spoke with the bicyclist involved in the accident, Timothy Murtagh (S49233464). Murtagh stated he was traveling Northbound on Centre Street on his Cannondale bicycle in the Southbound lane. Murtagh was not wearing any safety equipment while operating his bicycle (helmet) and was wearing a dark red jacket. Murtagh stated as he approached Wesley Street, MV1 "jammed on the gas" and hit him in the roadway. Murtagh stated he then fell to the ground and landed in the middle of the street. While speaking with Murtagh I told him it was reported to me by the operator of MV1 that he used a racial slur during their interaction. Murtagh acknowledged that he did and stated, "I called him a nigger". I observed moderate damage to the front wheel/rim of Murtagh's bicycle. Murtagh reported no injuries. Murtagh was advised and issued Massachusetts Uniform Citation T1443441 for Chapter 85, Section 11B (Fail to ride on the correct side of the roadway). Photos were taken at the scene and submitted to the IT Bureau. Due to the language used by Murtagh during his exchange with Stewart, Officer Rosenbaum of the Community Services Bureau was made aware of the incident for follow up.

On Tuesday, April 16, 2019 while assigned to Traffic unit N525, I responded to the area of Plymouth Road and Cochituate Road, Newton for a report of a motor vehicle/pedestrian accident involving a City of Newton vehicle. The weather at the time of the accident was clear and sunny. The road surface was dry. Plymouth Road and Cochituate Road are both public ways maintained by the City of Newton. The injured pedestrian, Officer Steven Emmanuel, was transported to Newton Wellesley Hospital prior to my arrival on scene. I spoke with the operator of MV1, Officer Justin Lau. Lau stated he was on duty in his official capacity as a Newton Police Officer while operating a City of Newton owned 2018 Ford Fusion (MA: 9PYM80, Newton PD #1319). Lau stated he was parked on Plymouth Road (N) at Cochituate Road and was speaking with Officer Steven Emmanuel. Lau stated Emmanuel was standing next to the front driver side door area of his vehicle in the roadway. Lau stated a large delivery trucked approached their location on Plymouth Road from Woodward Street and was attempting to make a right turn onto Cochituate Road. Lau stated Emmanuel directed him to back his vehicle up so the delivery truck could get by their location. Lau stated he cut the wheel of the vehicle he was operating to the right; and very slowly began to back up from his location. Lau stated he was attempting to back the vehicle up to get closer to the curbing and out of the roadway. Lau stated as the vehicle was in motion the front driver side wheel made contact with Emmanuel's left foot. Emmanuel's left foot subsequently became lodged in the front driver side wheel well area. Emmanuel fell to the ground in obvious pain. Emmanuel directed Lau to pull the vehicle forward in an effort to release his foot. Emmanuel's foot was freed and NPD Dispatch was notified of the accident. Pictures were taken of MV1 and submitted to the IT Bureau. I observed no damage to the front driver tire area. I spoke with Officer Steve Emmanuel in the Newton Wellesley Hospital emergency room where he was being evaluated for a left foot injury. Emmanuel stated at the time of the accident he was working a paid detail for Paramount Pictures on Plymouth Road in his official capacity as a Newton Police Officer. Emmanuel stated he was speaking with Officer Lau on Plymouth Road at Cochituate Road. Emmanuel was standing in the street on his post and Lau was sitting in MV1 (parked). Emmanuel stated a Fed Ex delivery truck approached their location in the roadway and was attempting to get by them. Emmanuel stated Lau began to back his vehicle up in an effort to get out of the way of the delivery truck. Emmanuel stated the front driver side tire of MV1 ran over his left foot. After the tire ran over his foot, it then became lodged in the wheel well and he fell to the ground. Emmanuel stated his foot was freed from the vehicle and he was transported to Newton Wellesley Hospital by Newton Medics for further evaluation.

On April 24, 2019, while assigned to N496, I was called by Dispatch to respond to the area Beacon Street and Hammond Street for a motor vehicle crash. This was called in by Boston College Police Department. Upon arrival MV #1 was on Beacon Street just past Hammond Street at the Island with considerable vehicle damage. MV#1 stated while traveling westbound on Beacon St, while approaching the intersection of Hammond he stated he started to feel strange and not like himself. MV#1 stated he then struck the curb with his front right tire, not believing his accident was very serious at this time. He reversed his vehicle off the sidewalk and proceeded to travel westbound on Beacon St. After passing through the intersection he stated his car felt very off at which point he initiated his hazards and pulled over to the right. MV#1 stated he has a history of epilepsy and currently takes Oxcarbazepine twice a day for his condition. He stated he did not believe he had a seizure or lost consciousness but did not recall the incident very well. Boston College Ofc. Cadogan witnessed the incident, and stated the following, while conducting a directed patrol at the intersection of Beacon and Hammond St he saw MV#1 traveling westbound on Beacon St. Ofc. Cadogan stated MV#1 then took a sudden tight turn on to the curb striking the metal fence surrounding the BC facility. MV#1 was then seen / heard revving his engine while still in drive before reversing his vehicle off the curb and attempting to continue on Beacon St westbound. The fence surrounding the BC Facility sustained moderate damage. Tire marks in the grass show the path of MV#1 in direct line to the damaged property. While on scene, I spoke at length with Vanhorne. I did not detect or any alcohol from his breath as he spoke. Vanhorne reported he only consumed one beer tonight at roughly 2030hrs and smoked a little marijuana at 2200hrs. Vanhorne's eyes did not detect any signs of alcohol and he walked with no signs of any impairment. Sgt. Devine administered a horizontal nystagmus test which revealed no signs of impairment. Vanhorne reported he was at a work event and then visited a friend in Brookline. Vanhorne reported his last seizure from epilepsy occurred a little over two years ago. Pictures of the scene were taken and submitted to IT to be attached to this report. MV#1 operator stated he was not injured at this time. MV#1 was towed from the scene by Todys's due to heavy front end damage. Vanhorne was picked up from the scene by Paul Miller his step father. A towed MV form was filled out and placed in the traffic box. Witnesses Cassandra Williams and Bill Peirce observed the crash and stated identical narratives to Ofc. Cadogan's. An Immediate Threat form was filed with the RMV (via Fax and E-mail at approximately 0650hrs 4/24/19)due to his medical history and his explanation with this crash. All streets mentioned are public ways in the City of Newton. It should be noted that the road was wet from rain on this night. Update: on 4/27/19 Sgt.Devine was able to watch the video of the incident from Boston College Police. The video confirmed the statements given by BCPD and witnesses's. Due to the aforementioned statements and the video surveillance of the incident, MV#1 Operator will be cited with Marked Lanes Ch89/4a (MA Uniform Citation #T1441792. Vanhorne will be notified via phone he will be receiving the ticket in the mail.

The operator of MV1(ma reg 773KV5) was traveling WB on River Street through the intersection of Cherry St when her vehicle was struck by a motor vehicle traveling on Cherry St. The operator of MV1 stated she had a green light and MV2 went through a red light striking her driver's side door. The operator of MV2 (MA reg 5CG678) stated she was traveling NB on Cherry St approaching the intersection of River St when she struck MV1. Operator of MV2 admitted she ran the red light and stated she zoned out for a minute. Operator of MV2 stated she was unsure if she wanted to turn onto River or continue straight onto Cherry and she ended up driving through the red light. Operator of MV1 was complaining of possibly head injury so I called for an ambulance. ALS 28 arrived on scene to evaluate and ended up transporting Operator of MV1 to NWH. Operator of MV2 stated she was not injured and did not require medical evaluation. Both vehicles sustained heavy damage and required tow's. Tody's arrived on scene on towed both vehicles. After MV1 was struck the operator ended up crashing into the retaining wall of the West Parrish Burial Ground. There was significant damage to the fence and wall. I had dispatch call for a traffic car to respond to the accident to take pictures since the Burial Ground is City Property. Traffic Officer Gaudet responded and took pictures of the vehicles and damage to the wall. The operator of MV2 was cited for CH 89-9 Fail to stop for a red light. MA citation (T1441437) was mailed to operator of MV2

Witness 1 stated that vehicle 1 was travelling northbound on Nahanton St. Vehicle 1 made a left hand turn, westbound onto Winchester St. During the turn the right side of vehicle 1 made contact with the crosswalk signal post. Vehicle one was traveling westbound on Franklin street and taking a right hand turn to head north on Waverly Ave when the female operator overturned the wheel and struck pole number 21 on the corner of Nonantum St and Waverly ave. The male passenger then stated for the female party to leave the driver seat of the vehicle. The male party took possession of the vehicle and drove off. Witnesses stated that a female was driving and a male party then took over possession of the car and drove off. Witnesses then wrote down the license plate of the vehicle. Officer Anderson responded to the operators residence to check on well being and spoke to the male party. The male party stated he left the scene and stated that his daughter was learning to drive and she only has her learners permit when she overturned the wheel taking a right hand turn and struck the pole. Male party stated he didn't think there was damage to the pole so he left the scene, since the female party was shook up. Pictures were taken and submitted to IT.

Operator 1 of MV 1 was stopped on Brookline St (E/B) to take a turn onto Ober Rd. A vehicle had let Operator 1 turn onto Ober Rd and while turning a Bicycle struck the passenger side of her vehicle causing minor damage. No injuries to Operator 1 or Passenger of the vehicle. No tow needed and no citation issued. The operator of the bicycle was traveling (w/b) on Brookline st passing the cars on the right side. The bicycle did not see that there was a vehicle stopped in traffic allowing MV 1 to turn onto Ober Rd. Minor injuries to the operator of the bicycle on hands and his right knee. Medics responded and got a signed patient refusal. Pictures were taken of the vehicle, operator of the bicycle and turned into IT for attachment to the report.

The OP. of MV#1 states she was travelling on Beacon Street eastbound with a steady green light and was making a right turn onto Chestnut Street. That's when there was a collision with Bicyclist#1. Bicyclist#1 was travelling eastbound on Beacon Street and approaching the intersection of Chestnut Street. He noticed a non-involved vehicle in the intersection stopped there trying to make a left turn. He observed MV#1 slowly moving around that vehicle and thought MV#1 was going to continue going straight on Beacon Street so he pedaled through. That's when MV#1 was actually making a right turn and they collided together. Bicyclist#1 sustained some mouth injury and unknown bodily injuries. His bicycle sustained some damage to the front wheel and right handle bar. He signed a patient refusal with the medics. The OP. of MV#1 gave Bicyclist#1 a ride and transported his bicycle home. Several pictures were taken of the damaged bicycle. No new damage seen on MV#1 to determine impact location.

On 05/02/19 while on patrol I observed a Traffic Light Post knocked over and resting against a damaged wooden fence. The pole appears to have been struck by a vehicle as there are tire marks leading up to the base. The pole is resting against a new six (6) foot tall unpainted wooden fence belonging to #127 Elliot St. There was no response at the residence so I left a note for the homeowner on the front door. Pictures were taken by Traffic Officer Wade and dispatch notified the light company- Daigle Light . I caution taped off the area and pole to make it safe for passers by. There is no information on the vehicle that struck the pole and a canvass of the area revealed no further leads.

On May 3, 2019 at approximately 18:18 I responded to a motor vehicle crash on Beacon St at the intersection of Hammond Pond Pkwy. It was reported that a school bus was involved with no passengers on board. Upon arrival, both vehicles were pulled over on the westbound lane of Beacon St. Both vehicles were turning left onto Beacon St. from Hammond Pond Pkwy, which has two left turn only lanes. MV1 was in the left lane, and MV2 was in the right. MV1 attempted to merge with MV2 on Beacon St. but misjudged the turn and drove into MV2's driver's side. MV2 was a school bus contracted by the City of Newton with no passengers on board. Photographs were taken and submitted to IT to be attached to this report. No reported injuries.

Narrative:
On Sunday, May 5th, 2019 at approximately 1315 hours while working the City of Newton detail for the Walk for Hunger I was standing at my assigned post at the intersection of Centre Street and Willow Street. At the time, the road surface was dry and the weather was cloudy. Traffic was particulary heavy as there were several streets in the area still blocked off for the Walk for Hunger and there were participants still walking the route in the area. My attention was drawn to the intersection of Centre Street and Bowen Street as there was a motor vehicle travelling southbound on Centre Street with their left turn signal on attempting to take a left turn on to Gibbs Street. Gibbs Street was temporarily blocked to non-residential motorists as there were several pedestrians in the road crossing over Gibbs Street. As the motor vehicle attempted to turn, there were several pedestrians in the crosswalk crossing over Gibbs Street. The vehicle had stopped halfway into the northbound lane on Centre Street blocking on coming traffic. The operator appeared to realize Gibbs Street was blocked and attemped to turn the vehicle back into the southbound lane to continue southbound on Centre Street. As the vehicle turned back into the southbound traffic lane, the vehicles rear end collided with Officer Richard Clements, who was operating NPD motorcycle unit 533. Officer Clements was traveling southbound on Centre Street and was passing the vehicles in the southbound traffic lane which were currently all at a standstill. Officer Clements had his emergency lights activated as he was on his way to remove traffic cones from the street to open up all the traffic lanes along the event route. Officer Clements was working the City of Newton detail for the Walk for Hunger assigned to the motorcycle to assist with traffic. Officer Clements front wheel, left side of the motorcycle and left leg collided with the rear end of the motor vehicle. The impact of the crash caused him to fall off the left side of the motorcycle and land on the street face first. At this time I immediately ran from my post to Officer Clements location and radioed dispatch that a motorcycle officer was down and that I needed assistance. As I approached Officer Clements he was face down, unconcious and breathing. I was able to roll Officer Clements into the recovery position and at this time Officer Clements regained conciousness and was able to speak and stated " What happened? What was that lady doing she turned right into me". I advised Officer Clements that he had just been involved in an accident and to stay still and that help was on the way. The operator of the motor vehicle identified as Helen Leibner (S52717988) stopped her vehicle immediatley after the crash and got out of her vehicle to offer her assistance. I advised Helen and several other witnesses and bystanders who were now crowding around Officer Clements and I to back away from the scene and to not move any of the vehicles involved. Officer Selig arrived on scene and was able to help crowd control until further help arrived. Helen and all witnesses were advised to stand by for police to take their statements. While tending to Officer Clements he was disoriented and made several attempts to move his body to which I had to physically hold him still to prevent possible further injuries. While speaking with Officer Clements I noticed blood coming from the inside of his mouth. Officer Clements was in a noticeable amount of pain. He complained of head, back and left leg pain. Officer Clements was wearing a yellow ANSI certified Newton Police reflective jacket and a Newton Police issued motorcycle helmet. Once the paramedics and fire personnel arrived I was able to speak to the operator of the motor vehicle and several other witnesses. Helen stated she was traveling southbound on Centre Street operating her blue 2014 Audi A4 (MA REG 213BC9). Helen states she was attempting to take a left turn onto Gibbs Street and activated her left turn signal. Helen states she had stopped halfway into the northbound lane as several pedestrians were crossing Gibbs Street in the crosswalk at the time. Helen states after the pedestrians cleared she saw the traffic cones blocking the street and realized the street was closed. Helen states she turned her vehicle back into the southbound traffic lane and did not see Officer Clements or the motorcycle until impact. Helen was not injured and her motor vehicle sustained moderate rear end damage. Her motor vehicle was able to drive away from the scene. Witness Adron Gordon (S90357161) stated he was participating in the Walk for Hunger and had just crossed over Gibbs Street when the crash occured. He stated he witnessed the Officer on the motorcycle with the emergency lights on traveling southbound on Centre Street behind the motor vehicle. The motor vehicle stopped in traffic attempting to turn left onto Gibbs Street. He states then the vehicle made a slight turn back into the southbound traffic lane and the motorcycle collided with the rear end of the motor vehicle. He states the Officer then fell off the motorcycle landing in the middle of Centre Street. Witness Elizabeth McCarthy (S00242346) and Katherine Regan (S43486601) stated they both were in the same vehicle together stopped in traffic in the northbound lane of Centre Street just before the set of lights at Centre Street and Bowen Street. Both witnesses stated they do not remember if the driver of the motor vehicle had the turn signal on but they did see her attempt to turn onto Gibbs Street. They stated they also saw the motorcycle with emergency lights activated travel southbound on Centre Street approaching behind the vehicle. Witnesses state the motor vehicle then suddenly turned back into the southbound lane and the motorcycle collided with the passenger side tailight of the motor vehicle. Both witnesses state the motorcycle attemped to turn away from the vehicle to avoid the crash but was unable to do so. Witness Nina Wang (S39460985) stated she was parked on the southbound lane of Centre Street just prior to the crash scene. Nina states she was in her City of Newton DPW marked vehicle with the hazards on as she was working to assist with traffic safety for the Walk for Hunger. Nina states she witnessed the motorcycle travel southbound on Centre Street and pass the vehicles stopped in the traffic lane. Nina states the motorcycle passed the traffic using the left side and had the emergency lights activated. Nina states the motorcycle was just passing through the green light at Centre Street and Bowen Street when the motor vehicle that was stopped at the lights attempting to take a left turn onto Gibbs Street. Nina states the vehicle then abruptly jerked her car to the right and collided with the motorcycles left side. Nina states she heard a loud crash and witnessed the Officer fall off the motorcycle and land face first onto the street. The motorcycle's final resting place was on its left side approximately two feet away from where Officer Clements was located. The motorcycle sustained moderate damage to its left side, including the front wheel well and undercarriage. The motorcycle left scuff marks on the street which were measured and photographed. Several photographs of the scene and damage to both vehicles were taken and submitted to the IT Bureau. The motorcycle was towed by Todys. Centre Street and Bowen Street are both public ways maintained by the City of Newton. The injured motorcyclist, Officer Clements, was transported to Saint Elizabeth Hospital with fire fighters on board. I was able to follow up with Officer Clements on the phone the next day who was admitted over night at the hospital. Officer Clements stated he has no recollection of the accident and can only remember driving down Centre Street to begin to pick up cones to open up all lanes of traffic. Officer Clements stated he suffered a brain bleed, head, back, left foot and right thumb injuries. A Massachusetts Uniform Citation was issued and mailed to Helen for Chapter 89 Section 7A, failure to yield to emergency vehicle, police motorcycle.

Bicyclist was traveling west on Beacon St coming down the hill towards Hobart Rd. He stated that he started braking when his light turned yellow and his brakes malfunctioned and he could not stop before Hobart Rd traffic got a green light. They did not exchange information and the operator stated that he could not wait for police to arrive. The bicyclist signed a medical refusal. Pictures were taken by me of the damaged bicycle. According to the bicyclist, there was no damage to the MV.

On Sunday May 19th 2019 at 03:32, I responded to a single car accident at the intersection of Washington Street and Park Street. Operator of MV1 stated she was alone in the car and was coming from the Newton Corner rotary attempting to turn onto Park Street. She stated that she misjudged the right turn and went up onto the first median knocking down a crosswalk signal pole and the pole with the traffic signal and Park Street sign. She then went up onto the second median taking down another crosswalk pole and a street sign pole. Operator of MV1 stated she was fatigued and was not paying attention when the accident occurred. She was evaluated by Cataldo Medic 1 and signed a patient refusal. David Donahue of Todys Towing removed the vehicle from the accident scene. Large debris and the poles were placed onto the median and out of traffic. 494 and myself placed cones over each of the bases of the poles. Dagle and the City of Newton were notified about the damage and for the debris to be picked up in the morning. A total of 12 pictures of the damage was taken and turned into IT.

On 05/20/2018 at 17:30 hours I was notified by Captain Anastasia of a motor vehicle crash involving car, and an electric scooter. Captain Anastaisa stated that the operator of the scooter was a 12 year old male who had sustained unknown injures and had been transported to Boston Children's Hospital via ambulance. Upon my arrival at the crash scene I made the following observations, Putnam and Winthrop streets are public ways in the city of Newton that are two way single lane non divided roadways. The roadway surface was wet from rain earlier in the day. Putnam St runs north to south and Winthrop St runs east to west. There is a stop sign at the end of Winthrop St at Putnam St. I observed MA reg 54MN41 a 2005 Buick LeSabre stopped in the south bound lane of Putnam St approximately 1/2 of the way into the intersection with Winthrop St. I observed approximately three feet of skid marks from the Buick's rear tires. I also observed at the crash scene a Segway Ninebot electric scooter that was on the ground near the Buick(cid:31)s rear passenger side door with its handlebars detached. I spoke with the operator of the Buick, Robert DeSouza who stated that on 05/20/2019 at approximately 16:50 hours he was driving south bound on Putnam St at approximately 25 miles per hour. DeSouza stated that he saw a man, later identified as Jose Estabil standing on the east sidewalk of Putnam St facing Winthrop St who was yelling and waving his arms at something on Winthrop St. DeSouza stated he looked to his right towards Winthrop St and his view was obstructed by a row of hedges. DeSouza stated that as he was looking to his right he began to apply his vehicle's brakes when he saw someone coming at me ". DeSouza stated that he heard a crashing noise from the rear passenger's side of his vehicle. DeSouza stated that he stopped his vehicle after the crash and saw a boy lying on the ground at the rear passenger side of his vehicle. DeSouza stated that he was not injured in the crash. DeSouza's vehicle had minor damage to its rear passenger side door, and did not require a tow. Witness Charlotte Hart stated that at approximately 16:30 hours on 05/19/2019 she was diving south bound on Winthrop St. Hart stated that she had to stop short on Putnam St to avoid a crash " with a boy on a scooter that was racing around on Winthrop St. " Hart sated that she told the boy on the scooter that he should not be ridding the scooter in the street. Witness Jose Estabil stated that he was walking on the east side walk of Putnam St across from Winthrop St. Estabil stated that he saw a boy an electric scooter on Winthrop St approaching Putnam St. Estabil stated that he saw DeSouza's vehicle driving south bound on Putnam St approaching Winthrop St. Estabil stated that the boy on the scooter made no effort to stop, slow or avoid the crash with DeSouza's vehicle. Witness Laurel Fransworth stated that she was driving north on Putnam St approaching Winthrop St when she saw the crash occur. Farnsworth stated a boy on an electric scooter on Winthrop St crash into the rear passenger side door of DeSouza's vehicle. Fransworth stated that she believed that the row of hedges at the end of Winthrop St obstructed DeSouz's vehicle. Rowan Delaney ( 02/27/2007 ) stated that on 05/20/2019 at approximately 16:00 hours he and his fellow F.A. Day Middle School student Thiaugo Rubino ( 03/04/2007 ) were having a race around the area of Winthrop St, Putnam St and Shawn St. Delaney stated that he was on foot running on sidewalks while Rubino was on an electric Ninebot Segway scooter. Delaney stated that Rubion was not wearing a helmet or any other safety equipment. I spoke with Delaney(cid:31)s father Royston Delaney, who stated that the scooter was his and that he was unaware that Rubino was using his scooter, and that he has never seen him ridding it before. Royston Delaney stated that the scooter is battery powered and has a maximum speed of 19 MPH. Royston Delaney stated that his son told him that only Rubino was ridding the scooter. Rubino was transported via ambulance to Boston Children(cid:31)s Hospital. On 05/20/2019 Rubino underwent surgery for a brain bleed. I spoke with Rubion(cid:31)s mother Sonia Rubino who stated that as of 05/21/2019 her son was under observation and in addition to the brain surgery her son had a broken arm, broken leg and a broken back. Based upon witnessed statements and my observations of the crash area I believe that Rubion was at fault for the cause of this crash. Myself and Officer Helms took photos of the crash area and the disks were turned over to the NPD(cid:31)s IT bureau for downloading.

The operator of MV1 stated he was taking a left onto Waldorf Rd. Westbound from Canterbury Rd. when he saw a child bicyclist traveling Eastbound on Waldorf Rd. in the street. He stated that immediately after, the solar glare from the sun impaired his vision and he could no longer see the child, and at that time the two collided. The left driver's side door area struck the bicyclist, as well as the possible front left tire. The operator and passenger stated they were uninjured. MV1 sustained little to no damage. The bicyclist was identified as CHO, Elena, and she stated she was traveling Eastbound on Waldorf Rd. approaching Canterbury Rd. She stated MV1 took a left onto Waldorf Rd. towards her and at that time she collided with the left side of MV1 and fell off her bike. The rear wheel of the bike was bent due to the collision. Elena stated that she was ok but had right hip pain. She was wearing a helmet, green jacket, blue shirt, black pants, and was riding a pink and white Diamondback bicycle (Serial # A1504026648). Her mother KIM, Kyung arrived on scene and signed a patient refusal with AMB1, and transported her to Harvard Vanguard Wellesley for an evaluation. I took photos of the scene and submitted them to the IT bureau to be attached to this report.

Operator of MV1 states at approximately 8:30am she was traveling northbound on Centre Street approaching Jefferson Street when MV2, operated by a male driver, cut in front of her and struck her left front bumper with his right rear bumper and fled the scene. MV1 sustained minor damage to the front left bumper. I attempted to contact Mr. Jones via telephone with negative results. At approximately 9:30pm, Officer Dragone responded to 109 Vernon Street in an attempt to raise Mr. Jones, but there was no one home and the vehicle wasn't in the driveway. On Thursday, June 6, 2019 while assigned to Traffic unit N525, I responded to the intersection of Waverly Avenue and Franklin Street for a report of an accident involving a City of Newton school bus. The weather at the time of the accident was clear and sunny. The road surface was dry. Waverly Avenue and Franklin Street are both public ways maintained by the City of Newton. The operator of MV1, Dorothy Wensink (S40227061), stated she was operating her 2010 Toyota Prius (MA: 538EV6) Eastbound on Franklin Street towards Waverly Avenue. Wensink stated she came to a stop at the stop sign on Franklin Street (E) at Waverly Avenue. Wensink stated as she began to travel through the intersection, a school bus traveling on Waverly Avenue (S) crashed into the rear driver side door area of her vehicle. Wensink reported no injuries on scene. I observed moderate damage to the rear driver side area of MV1. Wensink was able to driver her vehicle away from the scene. MV2 was a City of Newton contracted yellow school bus (MA BUN: 39173) transporting students attending the Ward Elementary School. The bus was returning to the Ward School from a field trip. The operator of MV2, Antonio Goncalves (S54275856), stated he was operating the school bus Southbound on Waverly Avenue towards Franklin Street. Goncalves stated MV1 abruptly accelerated across the intersection from Franklin Street. Goncalves stated he was unable to avoid making contact with MV1 and crashed into the rear driver side of the vehicle. I observed minor damage to the front middle and passenger side bumper area of MV2. The following Ward Elementary School student, teachers, and parents were traveling on the school bus at the time of the accident and reported no injuries. Students: Avishai Arbili, Eleanor Chen, Maeve Donovan, Esat Efendigil, Mark Elentuck, Julian Ismay, Rhea Khanna, Griffin Kirby, Ori Layani, Priscilla Le-Ngo, Callum McLaughlin, Carolina Muniz, Emily Ney, Hrithik Ponduru, Levi Rosenberg Van Gameren, Zachary Schwarzberg, Ava Smoller, Matthew Somers, Lillian Welford, William Balerna, Alexandra Bogdanova, Randi Green, Christopher Iatrou Johnson, Charlotte Nichols, Henry Pozen, Sofia Shadah Alcalde, and Zoey Steinberg. Teachers: Katherine Goldman and Lauren Ludman. Parents: Enrique Shadah, Brian Rosenberg and Xinxin Wang. The Principal of the Ward Elementary School, Becca Brogadir, responded to the crash scene and signed a patient refusal for all of the students with Newton Medics. The students, teachers, and parents, then walked back to the Ward Elementary School with Principal Brogadir. The school bus was able to drive away from the scene. Once the students arrived at the Ward Elementary School, the nursing staff there evaluated each child before they could return to class. The school staff stated they would make notifications to the parents of the children involved in the accident. After speaking with all parties involved, the operator of MV1 was cited in hand for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Pictures were taken of both vehicles and submitted to the IT Bureau.

On 06/08/2019 at 0150 hours I received a call from Sgt Lee in regard to a head on motor vehicle crash with entrapment that had occurred on Center St at the four way intersection with Cabot St and Sergeant St. Upon my arrival at the crash scene I was advised by Sgt Lee that all occupants of the involved vehicles had been transported to hospitals for treatment. I made the following observations of the crash scene , Center St is a two lane public way in the city of Newton that runs north to south. I observed a city of Newton contracted Cataldo ambulance stopped in the south bound lane of Center St at Cabot St. The ambulance was MA AMN registration AMB 370 a 2017 Ford Econo Line that had heavy damage to the front end and driver's side fender. I observed that the front tire of the ambulance was snapped off and was pushed back towards the driver's side door. I further observed that both of the front and side air bags of the ambulance were deployed. The front of the ambulance's final point of rest was 8'8" from a fixed overhead light pole on the northeast corner of Center St at Cabot St. The ambulance's front passenger side tire was 4'3" front the curb on the Cabot St side. The ambulance's rear passenger side tire was 2'3" from the Cabot St side curb. The ambulance's rear driver side tire was 5'1" from the double yellow line on Center St. The ambulance's front driver side tire was 6'3"from the Center St double yellow line. The width of Center St from the double yellow line to the Center St curb is 15'' and 16'" to the Sergeant St side. I observed a 2011 Mazda 3 with MA PAN registration 6KJ783 facing south bound in the north bound lane. The Mazda had heavy damage to it's front end, and I observed that it's front tires had snapped off. The Mazda's roof had been removed by Newton Fire to extract the occupant. The front of the Mazda's final point of rest was 40'3" from a fixed overhead light pole on the northeast corner of Center St and Sergeant St. The Mazda's front passenger tire was 4'6" from the double yellow line on Center St. The Mazda's rear passenger side tire was 7'6" from the Center St double yellow line. The Mazda's rear passenger side tire was 1'2" from the Center St double yellow line. The Mazda's front driver's side tire was on the curb on the Sergeant St side of Center St. There were no skid marks from the Mazda which indicates that no braking occurred. At the time of the crash, the Cataldo Ambulance was being operated by Paramedic Corey Bohan and Paramedic Tom White was in the front passenger seat. Bohan stated that he was operating his vehicle southbound on Center St towards Cabot St after clearing a medical call. There were no patients in the ambulance at the time of the crash, and Bohan estimated he was travelling 25 MPH . Bohan stated within less than two seconds he observed a vehicle traveling towards him northbound on Center St in the southbound lane and estimated it's speed to be approximately 80 to 100 MPH.Bohan stated he was able to slow to a near stop and had moved his vehicle towards the right shoulder on Center St before the Mazda struck his vehicle head on. Bohan stated that after forcing his way out of the driver's seat of his vehicle, he rendered medical aid to the operator and only occupant of the Mazda, Jessica Pondish. Bohan stated that Pondish speech was slurred and he detected an odor of an alcoholic beverage emanating from her breath and person. Bohan was later transported to Newton Wellesley Hospital for evaluation. The passenger in the ambulance, Tom White stated to officers that he was working on the ambulance's laptop computer and did not see anything leading up to the impact from the crash. White stated that he was able to force himself free of the passenger side of the ambulance, and rendered medical aid to Pondish. White stated that Pondish was " in and out of it " and did not appear coherent.White stated that he asked Pondish if she had consumed any alcoholic beverage and she nodded in the affirmative. White Stated that he was unsure if Pondish understood his questions. White stated that he did not detect any odor of an alcohol beverage emanating from Pondish's breath or person. White was transported to Newton Wellesley Hospital for evaluation. Pondish was freed from her vehicle after approximately 22 minutes by Newton Fire Department units by utilizing special extraction tools. Pondish was transported by Newton Medic Unit Two to the Brigham and Women's Hospital in Boston for treatment. From 06/08/2019 to 06/10/2019 Pondish had two surgeries and was unable to speak due to her condition. On 06/10/2019 myself and Officer Gaudet were able to speak with Pondish in her hospital ICU room at the Brigham and Women's Hospital. Pondish's parents Lorena and John Pondish were present while we spoke with her. Officer Gaudet asked Pondish about the events that led up to the crash. Pondish stated that on 06/07/2019 she was at a friend's residence in Norton, MA. Pondish stated that while at her friend's residence she consumed a bottle and a half of Riesling wine. After Pondish's admission myself and Officer Gaudet stopped our conversation with her. I read Pondish her rights directly from a Miranda card and explained them in full before allowing her to continue speaking with myself and Officer Gaudet about the events of that led to the crash. Pondish stated that she understood her rights. Officer Gaudet asked Pondish if she had consumed any alcoholic beverages prior to driving home to her residence in Newton. Pondish stated that she drank a bottle and a half of Riesling wine by herself between the hours of 1700 and 0000 hours on 06/08/2019. Officer Gaudet asked Pondish how large the bottles of wine was that she drank, and she stated that they were average size. Officer Gaudet asked Pondish how she perceived her state of being was after consuming a bottle and a half of wine. Pondish stated that she felt " buzzed " . Pondish stated that she left her friend's residence in Norton at approximately 0100 hours on 06/08/2019 to return to her residence in Newton. Pondish stated that she does recall the events leading up to the crash, except that she tried to make a phone call but the person did not pick up. Pondish stated that her seat belt was on and that she was not texting while operating her vehicle. Pondish stated that her injuries consisted of a broken left elbow and internal injures to her intestines. On 06/09/2019 I spoke to Cataldo Paramedic Robert Canning and Stephen Heirholcer who were the medic unit that transported Pondish to the Brigham and Women's Hospital. Canning stated that he rendered medical aid to Pondish for approximately 20 minutes while Fire units worked to free her. Canning stated that Pondish's legs were pinned under her vehicle's dashboard. Canning stated that during the approximately 20 minutes that he was in close proximity to Pondish he could detect a strong odor of an alcoholic beverage that was emanating from her breath and person. Canning stated that once Pondish was freed he continued to treat and monitor her during the transport to the Brigham and Women's Hospital. Canning stated that when he asked Pondish if she had consumed any alcoholic beverage she stated that she had. Paramedic Heirholcer stated that he did not detect any odor of an alcoholic beverage emanating from Pondish's breath or person. Based upon the physical evidence at the crash scene, and statements made to officers by all of the above mentioned persons, Pondish is being charged with violation of MGLs 90/24j OUI liquor, 90/24e negligent operation of a motor vehicle, 89/4a failure to stay within marks lanes, and 90/17 speeding. On 06/12/2019 at the conclusion of the crash investigation I issued MA Criminal Application T 1269996 in hand to Pondish at her hospital room and explained to her what action she needed to take with court. On 06/11/2019 a letter of preservation was sent to Brigham and Women's Hospital requesting that a sample of Pondish's blood, urine and medical records be saved. On 06/12/2019 Brigham and Women's Hospital confirmed that a sample of Pondish's blood had been preserved. As of 06/12/2019 a search warrant for Pondish's blood sample was pending. Photos of the involved vehicles and of the crash scene were taken and downloaded by the NPD's IT Bureau. Both involved vehicles were towed from the crash scene and towed motor vehicle forms were filled out. An Iphone and backpack belonging to Pondish were taken from her vehicle for safekeeping and returned to her on 06/11/2019. A copy of this report was faxed to the MA RMV Driver Control Unit to request an immediate threat suspension.

Operator of MV1 stated he was traveling Northbound on Walnut Street when MV2 was traveling Eastbound on Linwood Ave, crossing Walnut St. As MV1 approached the intersection MV2 was crossing and MV1 struck MV2. Operator of MV2 stated after she stopped at Linwood Ave and Walnut St, she was crossing Walnut St heading eastbound when MV2 came out of nowhere and attempted to swerve out of the way. MV2 struck the side of MV1 and proceeded to drive over the median up the embankment. MV1 had moderate damage to the front driver side bumper and rear driver side bumper. The operator was transported to Newton- Wellesley Hospital and a Daniel Kibugo (S23136964) took possession of the vehicle. MV2 had significant damage to the front bumper and hood. Also, damage to the passenger side, front and rear. MV2 was towed by Tody's. Photos were taken of the damage to the City Property and the SIM card was placed in the IT Mailbox.

OPERATOR #2 FURTHER STATED SHE LOOKED PRIOR TO CHANGING LANES AND NEVER SAW VEHICLE #1. PICTURES OF THE DAMAGED FENCE WERE TAKEN BY OFFICER MICHAEL BOUDREAU. On 06/26/2019, while assigned to N494, I, Officer Conary, responded to Cabot Street at Blake Street for a motor vehicle accident involving a City of Newton vehicle, fire truck. Operator of MV1 stated that he was traveling Northbound on Blake Street taking a left turn onto Cabot Street. Cabot Street is under construction and there was barrels in the road which caused MV1 to go around them. MV1 was to close to MV2 while taking the turn and hit MV2. Operator of MV2 stated that she was stopped Eastbound on Cabot street, waiting to take a right turn onto Blake Street. When MV1 was turning, it was too close and struck her vehicle. All parties were offered and declined medical attention. MV2 had to be towed from scene. Pictures were taken and will be submitted to IT accordingly.

The OP. of MV#1 states he was on the eastbound lane of Washington Street, waiting to make a left turn onto Beacon Street. When he saw there was a break in traffic, he made the left turn and did not see Bicycle#1 coming off the sidewalk, thus colliding with it. The OP. of Bicycle#1 states he was travelling westbound on Washington Street on the sidewalk. He states Washington Street westbound is too dangerous for him to ride on the roadway during rush hour traffic. As he was crossing Beacon Street, Washington Street westbound traffic still had a green light and that's when MV#1 collided into him while making a turn. The OP. of Bicycle#1 was wearing a helmet and had front and rear lights. He did not want me to take his bicycle so he brought it with him to the hospital to be picked up by a family member. The operator of MV#1 stated he was travelling northbound on Walnut St when the top portion of his vehicle caught some low hanging wires and took down 2 utility poles and damaged a third utility pole. There were no reported injuries to the operator of MV#1 and no visible damages to MV#1. The operator of MV#1 is a delivery driver that is familiar with the area and frequents the same route. There were several live primary wires down on Floral St and Walnut St. Unit N-498 Officer Raymond blocked off Walnut St at Centre St, Unit N-502 Officer Nardelli blocked off eastbound Floral St and I blocked off Walnut St at Lincoln St. The surrounding pedestrian sidewalks were all taped off with Police caution tape. MV#1 struck the low hanging wires on Walnut St at the intersection of Floral St. Utility pole #A (47/12 ) located on Walnut St directly in front of the parking of the "Orchid Dry Cleaners" at 1175 Walnut St was cracked at the base. Utility pole #B was located on the westbound side of Floral St was split at the base and was dangling over the parking lot at 1175 Walnut St. Utility pole #C was located on the eastbound side of Floral St and was completely removed from its foundation and was now leaning on the building at 1182 Walnut St. The street/ stop sign of Floral St was damaged with the "Stop" sign and the "Floral St" sign completely removed from the pole (Located on the eastbound side of Floral St). A city tree limb that was damaged and landed on top of parked Vehicle #2 (Parked on the eastbound side of Floral St across from 49 Floral St. Wisconsin registration 850TGM). The owner of vehicle #2 was informed of the damage. Traffic Officer Gaudet took digital photographs of the damaged street/ stop sign and the damaged city tree. The witness Pat O'Hara of 1185 Walnut St stated MV#1 did not cause the accident but a previous bigger truck did. All he could provide was that the vehicle was a big truck travelling northbound on Walnut St. The witness Edna Jacquet stated she was parked on Walnut St (Parked between 09:00 to 09:15) at the MBTA bridge facing southbound when she observed the accident. Edna stated she observed the operator make eye contact with her and she proceeded to honk her horn in attempts to stop him. Edna stated the vehicle that caused the accident was a white colored 18 wheeler type of truck with the markings of "3 letters" all in different colors. Edna have a company video camera that faces forward attached to her vehicle that may have captured the accident. I spoke to her company manager Lisa Sanella at "TransAction Corporate Shuttles" located at 5 Wheeling Ave in Woburn MA and she will notify me if she is able to view video footages. The building owner of 1170- 1176 Walnut St Stas Gavronsky have a surveillance camera that faces Walnut St southbound towards Floral St. He will review the video footages once power have been restored that the area and notify me. On 01/07/2020 Lisa Sanella contacted me stating that they were able to review video footages from the front facing camera of Edna's shuttle bus. She stated that the involved vehicle was described as a big white colored 18 wheeler type of truck and did not have a front registration plate but did display the commercial company name of "GHG Logistics". Lisa stated the accident was time recorded at 09:13. Lisa made a copy of the video footage and will attempt to email it or deliver a flashdrive to me. An internet search of GHG Logistics showed that it is a distribution company out of 6390 Hedgewood Dr suite #300 out of Allentown PA. This company distributes wholesale sushi to the northeast area and their delivery trucks are equipped with "Rydsmart" GPS tracking devices enabling them to know their locations at all times. I spoke to Joanna Gerwig who is the store support team leader of the "Wholefoods" located at 916 Walnut St inquiring about a possible sushi delivery and she confirmed that GHG Logistics does regular deliveries and did deliver items yesterday morning between 08:00 -09:00. She stated that the truck displayed trailer #250634-481801 and that the driver was described as a big sized white male by the name of "Will" with a contact number of (570) 350-3453. She also have video footages of the truck making the delivery and provided me with their contact number of (800) 668-7700 and their corporate number of (484) 223-4502 (Confirmed to be the same company as my internet search). I viewed the video footage from Transaction Corporate Shuttles and it showed the 18 wheeler truck was travelling northbound on Walnut, catching the utility wires above and causing an electrical spark on the eastbound side of Floral St. It showed the truck slowed down and then continued to travel northbound. I spoke to Will at (570) 350-3453 and he confirmed he was making deliveries to the Wholefoods and was on Walnut St at approximately 09:00. Will stated the top corner portion of his truck did catch the above wires and heard a "twang". Will stated he briefly stopped and did not observed any damages so he continued to make his deliveries. Will stated he have been making the same delivery route for 2 years and never had an issue with low hanging wires. Will's full name is William L. Gray and have a Pennsylvania driver's license # 24551299. The truck's registration is apportioned Indiana 2917618. I have forwarded the video footage to the I.T bureau and it was attached to this report.

Narrative:
On 01/09/20 at approximately 13:30 hours, a pedestrian Scott Lorenz came in to the lobby of Newton Police Headquarters to report that he was struck by a motorist while crossing the intersection of Cherry & Webster Streets. This accident took place at approximately 13:00 hours on this date. Mr. Lorenz further stated that he was walking on foot and attempting to cross Cherry Street going eastbound at Webster Street. He says that he did not activate the cross walk signal but thought it was clear to proceed as Webster Street had a red light signal. While attempting to cross, he says that the westbound lane of Webster Street received a green light and MV1 proceeded to turn left on to Cherry Street from Webster Street. He then looked at the motorist and thought that she saw him. Once she kept approaching and realizing that he was not seen, he started to back track but was struck by the front end of MV1. Lorenz then tumbled back and fell down to the ground. He says that he got up feeling fine and alerted the motorist that he was struck. The two of them exchanged information and then left the scene with out notifying 911 of the accident. After, Lorenz then came in to Newton Police Headquarters to report it. I observed a small round swollen red mark to the inside of his right knee area and a red mark to the back left side of his neck. Digital photos were taken of his injuries. Mr. Lorenz declined medical attention and wanted this incident documented in case his possible injuries worsened. I then spoke to the operator of MV1, Sharon Pywell (MA S23277423), on the phone after. Pywell stated that she was operating her Toyota Prius bearing MA reg. # 2197RW on Webster Street facing westbound at Cherry Street waiting for the green signal to turn left on to Cherry Street. Once the light changed, she attempted to turn and never saw a pedestrian trying to cross Cherry Street. I asked her if she ever felt contact to which she replied that she was not aware that she ever struck someone. She further says that once she did see him, she believed that he never lost his balance or ever fell down. She says that he retrieved his backpack from the ground and approached her driver's side opening her door stating that he was struck by her.

Operator of MV1 was traveling Westbound on Rt. 9 at a high rate of speed and veered off the road colliding with a street sign then landing in the backyard of 4 Meredith Ave damaging a fence. It should be noted MV1 failed to stop for police previously in a motor vehicle pursuit. MV1 sustained heavy front end damage. Operator sustained minor injuries and was transported to Newton Wellesley Hospital on a section 12. The vehicle was then towed by Tody's. A motor vehicle inventory form was filled out in regards. A message was left for the city to repair the street sign. Pictures were taken and placed into property and evidence. It should be noted a fence had to be removed at 4 Meredith Ave in order to removed the vehicle. I was able to get permission by the owner of the home to remove the fence.

On January 16, 2020 Officer Bergdorf, Sergeant M Wade and I, Officer Guarino responded to Union St @ Langley Rd for a report of a motor vehicle crash into a bicyclist. Upon arrival, both the operator of MV1 and the bicyclist were inside of 45 Union St, "Baramor" to get out of the cold. I spoke with the bicyclist who was complaining of left wrist and left hip/ tailbone pain. The bicyclist was driving Southbound on Langley Rd on the sidewalk on his black bicycle with no lights. He said that as he was traveling on the sidewalk on Langley Rd, crossing the intersection of Union St (just outside of the crosswalk), he was struck on his right front of the bicycle causing him to fall on his left side. The bicyclist was not wearing a helmet and did not have any lights on his bicycle. He was wearing a blue jacket and dark pants on a black bike. The operator of MV1 spoke with Sergeant M. Wade and Officer Bergdorf. MV1 said that he was traveling East on Union St and stopped at the stop sign and did not see any pedestrians or traffic coming. He proceeded across Langley Rd and struck the bicyclist with the front driver's side of his vehicle. Officer Bergdorf took photos at the scene, which were submitted to IT to be attached to this report. The bicyclist was transported to Newton-Wellesley for medical treatment. His bicycle was transported to his house and secured by Officer Larson.

On Thursday, January 23, 2020 while assigned to Traffic unit N525, I responded to the intersection of Brookline Street and Hartman Road, Newton for a report of a motor vehicle/pedestrian crash involving a City of Newton Crossing Guard. The weather at the time of the crash was overcast. The road surface where the crash took place was dry. Brookline Street and Hartman Road are both public ways maintained by the City of Newton. On arrival, the injured pedestrian was in the process of being transported to Newton Wellesley Hospital with minor injuries. I spoke with the operator of the vehicle involved in the crash, Menachem Uminer (S43996134). Uminer stated he was operating his vehicle on Brookline Street (S) towards Hartman Road. Uminer stated he was stopped in traffic on Brookline Street prior to the crash. Uminer stated the vehicle in front of him proceeded to travel straight from it's stopped position and he then began to follow the traffic on Brookline Street. Uminer stated he did not see anyone in the vicinity of his vehicle as he began to travel straight from his stopped position. Uminer stated as he was traveling by Hartman Road, the front passenger side area of his vehicle made contact with a pedestrian on the right side of Brookline Street at Hartman Road. Uminer stated he did not see this party until after they were struck and he then proceeded to pull his vehicle over. Uminer stated he was operating his vehicle at a very low speed at the time the collision took place. After speaking with Uminer, I observed the white 2017 GMC Yukon (MA: 9NX525) involved in the crash parked on Brookline Street (S) just past Hartman Road. Uminer stated the vehicle had been moved after the collision took place. I observed no obvious damage to the body of the vehicle on the front passenger side area where the collision took place. The front passenger door mirror was folded inward towards the door. Uminer stated he believes the front passenger door mirror was the point of contact between his vehicle and the pedestrian. I spoke with a witness to the crash, Ashley Garcia (S94193596). Garcia stated she was attempting to cross Brookline Street at Spaulding Lane. Garcia stated she entered the marked crosswalk and was a few steps into Brookline Street when a black colored vehicle traveling on Brookline Street (N) slammed on its brakes near the crosswalk. Garcia stated she looked to her left at the vehicle and then heard a thud to the right from the area where the crossing guard was standing near Hartman Road. Garcia stated she looked over and saw a crossing guard on the roadway on Brookline Street (S) at Hartman Road. The injured pedestrian involved in the crash is Cassandra Alexander (S23636307). Alexander is a crossing guard for the City of Newton and is assigned to the intersection of Brookline Street and Hartman Road for the Memorial Spaulding school crossings. This crash occurred while Alexander was performing her official duties. Alexander was wearing her department issued yellow reflective jacket at the time of the crash. I spoke with Alexander at Newton Wellesley Hospital where she was being treated for her injuries. Alexander stated she was performing her duties as a crossing guard at the intersection of Brookline Street and Hartman Road. Alexander stated she had just finished crossing two pedestrians in the marked crosswalk on Hartman Road when observed a pedestrian enter the crosswalk on Brookline Street at Spaulding Lane. Alexander stated she began to enter the intersection of Brookline Street at Hartman Road with her hands up and her department issued yellow reflective jacket on in an effort to stop traffic in the area to allow the pedestrian in the marked crosswalk safely cross. Alexander stated she was struck on her left side by a passing vehicle. Alexander stated she did not see the vehicle and fell to the roadway. Alexander stated as she was falling to the roadway she extended her right arm in an effort to brace her fall. Alexander stated she had injuries to her arms and legs and was sore from the collision. I spoke with a second witness to the crash via telephone, Anand Vaidya (S43996134). Vaidya stated he was stopped in traffic behind MV1 on Brookline Street (Southbound). Vaidya stated he observed the vehicle in front of MV1 proceed straight on Brookline Street after being stopped in traffic. Vaidya stated he then observed MV1 proceed straight on Brookline Street at Hartman Road at a low rate of speed. Vaidya estimated the speed of MV1 at this time to be 5 miles per hour. Vaidya stated a crossing guard was on the right side of the roadway on Brookline Street at Hartman Road wearing a yellow and black reflective jacket and snow pants. Vaidya stated the crossing guard had her back towards him and MV1, and extended her arm out to stop traffic. Vaidya stated at this time MV1 rolled by the crossing guard at a low rate of speed and the front right fender area of the vehicle hit the left rear leg/lower back area of the crossing guard. Vaidya stated the crossing guard bounced off of the car and fell to the roadway. Vaidya stated he believes MV1 was unaware he hit the crossing guard and observed the vehicle travel another 10 feet until it came to stop on Brookline Street. Vaidya stated the crossing guard was not in a marked crosswalk at the time of the crash. After speaking with all parties involved, Uminer will be cited with Massachusetts Uniform Citation T0115905 for Chapter 90, Section 14 (Failure to slow for a pedestrian in the roadway). Photos were taken of the vehicle and scene and submitted to the IT Bureau.

Vehicle #2 suffered minor damage to it's front end driver side. Both Vehicles were towed by Todys. On Thursday, February 6, 2020 while assigned to Traffic unit N525, I responded to the intersection of Franklin Street and Shornecliff Road, Newton for a motor vehicle crash involving a City of Newton owned vehicle. The weather at the time of the accident was light rain. The road surface was wet. Franklin Street and Shornecliff Road are both public ways maintained by the City of Newton. I spoke with the operator of MV1, John Fleming (S67400731). Fleming works for the Utilities Division for the City of Newton and was operating a City of Newton owned 2005 INTL 700SER (MA MVN: M98492 ) at the time of the crash. Fleming stated he was traveling on Franklin Street (N) at the intersection of Shornecliff Road. Fleming stated he went to back his vehicle up and did not see any vehicles behind him. Fleming stated the real of his vehicle crashed into the front of MV2. I observed damage to the sand dispensing attachment on the rear of MV1. Fleming reported no injuries. I spoke with the operator of MV2, Lisa Perry (S21890600). Perry stated she was operating her 2016 Honda Pilot (MA: 5WP321) on Franklin Street (N) towards Shornecliff Road. Perry stated she was stopped behind MV1 when it backed up and crashed into the front of her vehicle. I observed damage to the front bumper/light area of MV2. Perry reported no injuries. A query of Perry's registration status through Newton Dispatched stated it was expired. Perry was able to renew her registration on scene and was issued Massachusetts Uniform Citation T2013858 in hand for Chapter 90, Section 9 (Expired Registration). Photos were taken of both vehicles and submitted to the IT Bureau.

On 2/12/2020 at approx 0725hrs while assigned to 497 I responded to the area of 57 Metacomet Rd for a report of a mv crash without injuries. upon arrival i met with the operator of Ma Reg V36615, Riberioa DASILVA who stated while traveling EB on Beacon St he suddenly lost his brakes , swerved to the left causing tracks through the wet dirt on the berm , travelling across the front lawn of 57 Metacomet leaving no damage coming to a stop on Metacomet Rd. DASILVA arranged for Nicks Towing to take his car. Mass Uniform Citation T2015309 issued to DASILVA for violation MGL 90/26A Failure to Notify RMV of Address Change. Photos of damage taken and submitted to IT.

It appears to have been struck overnite by unknown vehicle. Dispatch reported that the r/p, heard a loud noise at around 03:00 hours. However never called the police. Pictures taken. Camera disc was placed in the Technology mailbox. Dagle electric light company notified.

Narrative:
On 02/26/2020 I responded to the area of Eastbourne Rd and Ireland Rd for a motor vehicle crash involving a pedestrian. Eastbourne RD is a public way which runs from Ward St north bound intersecting with Ireland Rd on the far north end. Ireland RD is a public way which runs west to east. Both the east end and the west end of Ireland Rd are dead ends. The crash occured on Ireland Rd's east end near the intersection of Eastbourne Rd. The crash involved Vehicle 1 described as a 2016 Acura SUV color grey bearing MA passenger registration 3RB127. Vehicle 1 had a valid inspection sticker which expires on 08/30/2020. Vehicle 1 was operated by its owner identified as Barbara Ball. Ball has an active MA Operators License (S18727836). The involved pedestrian was identified as Bruce Terrio. Terrio is an employee of Feeney Brothers Utility Services. Feeney Brothers Utility Services is a contractor of National Grid Gas Company. Upon arrival I observed the following: The crash occurred in the daylight hours around 10:30am on Ireland Rd near the intersection of Eastbourne Rd. At the time of the crash the weather conditions were clear, 48 degrees Fahrenheit and overcast skies (weather.gov). The area where the crash occurred was an active construction site for Feeney Brothers Utility Services. The area of the crash is a dead end. The area is accessed from Ward St from either Eastbourne Rd or Coolidge Rd. The entrances of both Eastbourne Rd and Coolidge Rd had construction signs indicating utility work in the area. At the time of the crash, all roadways were open to vehicle traffic. On the eastbound side of Eastbourne Rd just prior to the intersection of Ireland Rd, I observed a large air compressor which was parked in the roadway facing south bound. No traffic cones were located near the air compressor. To the right of the air compressor were thick green bushes. A wooden fence was located to the right side of the green bushes. On the east side of Ireland Rd I observed a large excavated hole on the right side of the roadway. The hole was near the intersection of Eastbourne Rd and was located approximately three feet from the curb. I observed four traffic cones, two on the front side and two on the left side of the hole. Located on the front edge of the hole was a tire print. Vehicle 1 was located on the west end of Ireland Rd. The front passenger tire of Vehicle 1 had fresh dirt located on the sidewalls and the tire tread. Vehicle 1 appeared to have no visible damage. The pedestrian (Terrio) was being treated by EMS and Fire for injuries. Terrio was wearing brown pants and a green reflective safety vest. I spoke with the operator of Vehicle 1 Barbara Ball. Ball stated that she was operating Vehicle 1 northbound on Eastbourne Rd near the intersection of Ireland Rd. Ball stated that she made a right turn onto Ireland Rd and drove on to the edge of the excavated hole. Ball stated that her vehicle then made impact with Terrio who was in the hole. Ball stated that she immediately stopped, backed out of the hole and called 911. Ball stated that prior to making a right turn onto Ireland Rd, there were no traffic cones in the roadway or next to the hole. Ball stated that as she made the right turn onto Ireland Rd she did not observe Terrio or the hole until it was too late. Ball states that she resides at 80 Eastbourne Rd which is the residence located to the immediate right of the crash location. Ball states that she had been recently parking on Ireland Rd due to ongoing construction at her residence. Ball states that she was traveling at a slow speed prior to impact. At the time of the crash there were three other Feeney Brothers employees working at the site. These workers were identified as Steven Gamble, Mike Cabrinma and Gerard McComaskey. At the time of the crash all three workers were located further up Eastbourne Rd near the intersection of Coolidge Rd and did not observe the crash. At the time of the crash, Newton Police Sergeant Glenn Chisholm was working a paid detail for Feeney Brothers. Sergeant Chisholm stated that he did not observe the crash.Terrio was transported by Cataldo EMS for minor injuries. Several photos were taken of the scene. Photos were taken of the hole with traffic cones and without traffic cones. All photo's were submitted to the IT Bureau. After clearing the scene, I responded to St Elizabeth's Hospital in Brighton, MA to interview Terrio. Terrio stated that at the time of the crash he was standing in the hole on Ireland Rd performing gas utility work. Terrio stated that the front passenger side bumper of Vehicle 1 struck him in the right shoulder. Terrio stated that the impact caused him to lose his balance and fall down in the hole. Terrio states that after falling down he was able to climb out of the hole without assistance. Terrio stated that at the time of impact he was wearing a white hard hat which he thinks struck the pavement on the edge of the hole. Terrio states that at the time of the crash he was working alone in the hole. Terrio states that at the time of the crash traffic cones were located near the hole. Terrio stated that as a result of the impact he sustained minor injuries to his right shoulder and back. I then observed Terrio's hard hat which appeared to have a fresh scrape mark on the top. The hard hat also had broken interior straps. Terrio stated that the straps broke when he was struck by Vehicle 1. Photos were taken of the hard hat and were submitted to the IT Bureau. After reviewing all facts of this incident, Vehicle 1 appeared to have been traveling at a slow speed prior to the crash. This was evident due to the lack of tire skid marks prior to the tire print on the edge of the hole. It also appears that Ball's view of the hole and Terrio may have been obstructed by the parked air compressor on Eastbourne Rd. The thick green bushes and a wooden fence to the right of the air compressor also added to the obstructed view. The hole was also located in close proximity to the air compressor which reduced Vehicle 1's reaction time. Jeff Oliver of OSHA was also notified of this incident. Operator of MV1 came into the front desk of NPD HQ to report an past hit and run accident that occurred at 15:50 at the intersection of California and Crafts Street. Operator of MV1 stated she was travelling southbound on California Street and was stopped at the stop sign waiting to turn right onto Crafts Street. She was rear ended by the operator of MV2. Operator of MV1 got out of her vehicle, observed minor damages to her rear bumper and asked Operator of MV2 for her license and registration information to exchange info. Operator of MV2 said "you are fine, I am not giving you my information and I'm leaving now." Operator of MV1 was able to take photos of MV2's license plate. I left a voicemail for Operator of MV2 to call me back in regards to the accident. I have yet to hear from her. Both vehicles had very minor damage and neither party was injured. Traffic Bureau update (Officer Gaudet): On Wednesday, March 4, 2020, I attempted to contact the registered owner of MV2 with a negative result. A Hit and Run inquiry was mailed to the registered owner of MV2, Danielle Mcalpine (S64767445) On Wednesday, March 11, 2020, I made contact with Mcalpine. Mcalpine stated MV1 was in front of her in traffic. Mcalpine stated both cars were inching up from stop and MV1 slammed on her brakes. Mcalpine stated did not believe the vehicles made contact. Mcalpine stated the operator of MV1 got out of her car and yelled at her. Mcalpine stated she saw no damage to either car. Mcalpine stated MV1 got really aggressive and kept yelling at her. Mcalpine stated she provided her license place to MV1 and left the area. Mcalpine stated her insurance company is aware of the incident.

Based on observations and statements made, the following occurred. M/V#1 was traveling south on Dedham St. aproaching Countryside Rd. when the operator observed M/V#2 on the side of the road (just past Countryside Rd) start backing up. Operator #1 came to a complete stop, approx. 10 ft before Countryside, and saw M/V#2 still backing up. Operator #1 began to beep his horn as M/V#2 got closer, but it did not stop. The left rear of M/V#2 collided with the right front of M/V#1, causing damage. M/V#2 then drove off westbound on Countryside Rd. Operator #1 was able to take a picture with his cell phone of M/V#2's license plate (MA REG 5CH377). Dispatch was able to speak to the owner of M/V#2, who stated that her husband was driving. Andrei Rumiantsev Operator #2) arrived at NPD at approx. 22:00 hrs, with his wife (Owner M/V#2) acting as a translator, stated that he thought he heard something, but was unaware that he hit anything. Operator #2 was issued citation #T2080745 C.O. 19-15, Failure to use caution, when starting, stopping, turning, or backing.

On Sunday, March 1st 2020, at approximately 5:38pm, I, Officer Brooks, was dispatched to the intersection of Linwood avenue and Watertown street for a bicyclist struck by a MV. Upon my arrival I spoke with the bicyclist who stated she was traveling westbound on Watertown street, and was in the crosswalk at the intersection of Linwood avenue, when she was struck by MV1(MA REG67FD40), which had been traveling southbound on Linwood avenue. The bicyclist stated that the operator of MV1 did not come to a stop at the stop sign. The bicyclist was complaining of minor leg pain, but signed a patient refusal with Cataldo. The bicyclist was dressed in black pants, and a gray sweater, and was not wearing a helmet. The bicycle sustained damage to the handlebars. At the time of the accident the sun was setting, however the street lights had not yet come on. I then spoke with the operator of MV1 who stated she was traveling southbound on Linwood avenue, and was attempting to turn right onto Watertown street when she struck the bicyclist. The operator stated she never saw the bicyclist before the collision. The front of MV1 sustained minor damage. I was able to speak with a witness to the accident over the phone after clearing the scene. The witness stated that MV1 failed to stop at the stop sign at the intersection of Linwood and Watertown street, and that the bicyclist was already well into the crosswalk at the time of the collision. Officer Howes took pictures of the accident scene that have been submitted to the IT bureau. Based of the witness statements MA Uniform citation T2080576 has been mailed to the operator of MV1 for MGL C.89 S.11 Failing to yield for a pedestrian in a cross walk.

On Friday, March 6, 2020 while assigned to Traffic unit N525, I responded to the area of the Countryside School (191 Dedham Street, Newton) for a report of a motor vehicle crash involving a City of Newton contracted school bus. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with the operator of MV1, Gianna Gemelli (S95586165). Gemelli stated she was traveling Southbound on Parker Street towards the Boylston Street (E) on ramp in a JSC Transportation owned 2016 Dodge Grand Caravan (MA SPN: 105641). JSC Transportation is contracted by the City of Newton to transport students through out the city. Gemelli stated as she was traveling on Parker Street, MV2 attempted to pass her on her driver side to take a left onto Boylston Street (E). Gemelli stated the front passenger side of MV2 made contact with the rear driver side of MV1. I observed minor damage to the rear driver side bumper area of MV1. Gemelli was transporting two students to the Countryside Elementary School at the time of the crash. The students in the vehicle were identified as Mayson Martinez and Hailey Nesvacil. No injuries were reported by any of the occupants of MV1. Countryside Principal Elizabeth Herlihy signed a patient refused for Mayson Martinez. Martinez's mother Rosa Mordan was notified of the crash by school staff. Gemelli signed a patient refusal for her daughter, Hailey Nesvacil. Regina Moody was notified of the crash. I spoke with the operator of MV2, Clary Binns (s100112165). Binns stated she was traveling Southbound on Parker Street towards the Boylston Street (E) on ramp. Binns stated she attempted to go around MV1 to enter the on ramp to proceed onto Boylston Street when the front passenger side of her vehicle made contact with the rear driver side of MV1. I observed minor damage to the front passenger side fender/wheel area of MV2. Binns reported no injuries. Prior to my arrival, Binns stated she had two passengers in her vehicle at the time of the crash that had left the scene. They were identified as Maya Bock and Jonatan Freedman-Naditch. I contacted parents for both parties to notify them of the crash. No injuries were reported. Binns, Bock, and Freedman-Naditch are all students the Meridian Academy in Boston. Clary Binns was provided with Massachusetts Uniform Citation T0115909 for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Photos where taken of both vehicles and submitted to the IT Bureau. Parker Street and Boyston Street are both public ways in the City of Newton.

Operator of MV1(MA commercial reg. S29929) states he was driving westbound on River Street when multiple emergency vehicles approached the rear side of MV1. Operator of MV1 pulled over right away. The top of MV1 struck a city owned tree causing it to fall and lay across the road obstructing the westbound lane. It should be noted that MV1 is a box truck and visibility is obstructed. It should also be noted i was on scene at another car accident just prior to this one. Multiple emergency vehicles were in fact dispatched and responded to the other accident which corroborates with operator MV1's story. I have attached photos of the accidet to this report

On March 16th, 2020 at approximately 09:38 hours while working N491 I responded to a report of two car MV crash at the intersection of Newtonville Ave at Lewis Terrace. On my arrival I observed a large while box truck(vehicle1 ) with passenger side front end damage parked on Newtonville Ave just west of Lewis Terr. The other involved vehicle(#2) was located inside the construction site at #255 Newtonville Ave. I spoke with the operator of the Truck(veh#1) who reported that he was driving W/B on Newtonville Ave when vehicle #2 pulled out into the intersection from north side of Lewis Terr striking the front passenger side of his truck. The impact from the crash caused vehicle #2 off the road, through a chain link fence and into the construction sit to #255 Newtonville Ave before coming to a stop. There were numerous construction worker on the job site but miraculously none of them were struck by vehicle #2 as it crashed through the fence onto the site. I later spoke with the operator of vehicle #2 who was not injured. he stated he was driving S/B on Lewis Terr. and was entering onto Newtonville Ave when he was struck by vehicle #1. Operator couldn't remember if he looked for oncoming traffic prior to entering the intersection. I spoke with the son of Mr Goldfinger(operator #2) who had arrived on scene and he assured me his father would no longer be driving again. I cited Mr Goldfinger for failing to yield/stop(c89/c9) at an intersection. Ticket # t1444075 Todys towed vehicle#2 from scene and there were no reported injuries. Witness stated she was behind vehicle #2 on Lewis Terr.and observed him slowly enter out into the intersection without coming to a complete stop. Pictures taken by Construction foreman. All insurance information and drivers info from operator #2 was taken by him also.

On 3/17/20 while working N493, I responded to the intersection of Auburndale Ave and Newell Rd for an MVA car vs pedestrian. On arrival I located pedestrian Stephen Dugas DOB 8/25/50 sitting in the middle of Newell Rd bleeding profusely from the top of his head, but otherwise consious/alert. The driver of the motor vehicle (MA reg 93Y880) was still on scene. He is identified as Ali Alnamos DOB12/10/59. While the medics were tending to Dugas I obtained a statement from Alnamos. He stated that he was driving on Auburndale Ave towards Lexington St and went to turn right onto Newell Rd. Upon turning, he made contact with the pedestrian with what he believes to be his passenger side mirror. Alnamos was distraught while speaking with me, and told me that he did not see the man. It should be noted that there is a clearly marked pedestrian crosswalk on Newell Rd, right at the location of this accident. There are street lights in the vicinity and Dugas was wearing darker clothing. Medic 2 transported Dugas to St. Elizabeth's due to his head injuy. I responded to the St E's ER Department and spoke with Dugas shortly after he was transported. Dugas reports that he was out for a walk on Auburndale Ave and was heading towards Auburndale Square. He went to cross Newell Rd and was hit by a vehicle shortly after entering the crosswalk. Dugas is not sure what part of the vehicle made contact with him but states that he did not go over the hood. Newell Rd intersects Auburndale Ave only on the westbound side. Several pictures were taken of the scene and have been submitted to IT. I did not observe any damage to the vehicle involved, therefore a tow was not needed. Alnamos stated that he was not in need of medical attention. Due to Dugas' statement at the hospital, I am mailing Alnamos MA citation T2079194 for 89/11 Fail to Yield to Pedestrian in Crosswalk.

On 03/18/20 at approximately 13:01 I responded to the intersection of Winchester St and Heatherland Rd. for a two vehicle crash. Upon arrival I spoke with Newton Police Detail Officer Schaefer who was involved in the accident. Officer Schaefer was operating his own vehicle (Mass Reg#1JP123 Blue 2001 Cadillac Deville) and headed back to a paid Feeney Brothers detail. Officer Schaefer stated he was traveling due South down Winchester St. approximately 30mph when a light colored vehicle (later determined to be Mass Reg#9FG942- Grey 2010 Toyota Prius operated by Rosana Cristina Brasileiro) pulled out of the one-way Heatherland Rd, the wrong way. Officer Schaefer stated the Prius struck him on his drivers side front and rear doors causing the vehicle to crash into the curb and causing his side air bag to deploy. Officer Schaefer was wearing his seatbelt but struck his head off the interior of his vehicle. I attempted to speak with the operator of the other vehicle Rosana Brasileiro but she only spoke Portuguese. Newton Fire Department Engine #7 was also on scene and Fire Fighter Ian Moniz stated he spoke a different dialect but was able to communicate with her. Fire Fighter Moniz translated for me and I was able to ascertain that Brasileiro stated she was driving due West on Heatherland Rd(wrong way) when she didn't see the other vehicle and made contact with it. I asked Brasileiro for a License and she handed me a Brazilian Passport and stated she didn't have a license. She stated she was just visiting from Brazil and that she was operating her bosses vehicle. Brasileiro was handed Mass. Criminal Citation #T2080843 for Ch90/ Sec10- Unlicensed Operation, Ch90/ Sec 16-Operating on a one way the wrong way and Ch 89/ Sec 8-Failure to yield to the right of way. Both vehicles were towed from the scene while Officer Schaefer went with the Medics to Newton-Wellesley Hospital to get checked out. Sgt. McLean and Traffic Officer Wilson (N523) were also on scene and Officer Wilson took pictures. Sgt. McLean traveled to N.W.H. to check on Officer Schaefer. All roads mentioned/traveled on in this report are public ways in the City of Newton as they are serviced and maintained by this city.

Based upon statements made to me by both operators and my observations of the crash site and the involved vehicles, I issued operator of vehicle two MA Citation T2015987 and cited him for a violation of City of Newton Ordinance 19/75, failure to use care while entering a lane of traffic. I took photos of both vehicles and the disk was downloaded by the NPD's IT bureau. All roadways in this report are pubic ways in the City of Newton.

On April 16, 2020, while assigned to WN, I took a report at the front desk for a past MVA/Bicyclist hit and run. LISTUNOV, Oleg reported that on 4/15/20, at approximately 2000 hrs, he was traveling on his bicycle Northbound on Chestnut St. approaching the intersection of Boylston St. He stated he did not stop at the stop sign due to the hill decent, at which time he was struck by MV1 on the left side and knocked to the ground. MV1 was traveling Eastbound on the Boylston St. off ramp. LISTUNOV stated MV1 stopped at the stop sign, and probably did not see him as they then proceeded straight into the intersection. LISTUNOV stated the female driver asked if he was okay and then left the scene. He wasn't able to remember any description other than a possibly yellow, orange, or light colored car. LISTUNOV then proceeded home before responding to Brigham and Women's Hospital. He sustained the following injuries: abrasion to the left side of forehead with internal bleeding, left black eye, right dislocated pinky, cut under chin, abrasions and swelling on both knees. I took photos of his injuries and submitted them to the IT bureau. LISTUNOV was riding a black bicycle, and was wearing black jeans, blue jacket, blue baseball hat, and no helmet.

On Sunday, April 14th 2020 at approximately 1811 hours I, Officer Newton, was sent to Boylston St at Margaret Rd for a vehicle that hit a stop sign and mailbox and proceeded down Margaret Rd. The RP was able to get the plate of the vehicle (MA Reg 2NHW21) which came back to the address of 60 Margaret Rd. Upon arrival Ofc Hatfield located the vehicle with front end damage and was speaking to the operator. The operator stated he was taking a right turn from Boylston St onto Margaret Rd and took the turn too wide and hit the USPS mailbox and the stop sign. He stated he was scared so he kept driving to his house. Pictures of the stop sign, parking sign, and mailbox were taken and submitted to IT. The vehicle was parked legally on the street and was not needed to be towed. Operator had minor cuts on his hand but declined medical attention. Operator was cited with MA Citation T2012820 for 90/24 Leaving The Scene of Property Damage and 89/4 Marked Lanes Violation.

Based on observations, and statements made, the following occurred. M/V#1 was the second car in line, in the westbound lane of Elliot St. The operator stopped behind a white M/V that had stopped for a red traffic light. When the light turned green, M/V#1 followed the white M/V into the intersection of Elliot St. and Chestnut St. when it was impacted by M/V#2 (which was traveling north on Chestnut St.). Based on witness accounts, M/V#2 was traveling west on Chestnut St. at a high rate of speed, and ran the red light. The impact forced M/V#1 into a traffic light pole on the south west corner of Chestnut and Elliot St. knocking it down. Operator #1 was treated on scene by medics, Operator #2 was transported to NWH. Citation # T2080748 was issued to Operator #2 for MGL CH 89 S 9, Red Light Violation Ditigal pictures were taken of the damage to the vehicles, and city property. The memory card was placed into the IT beurue.

Operator of MV1 stated she was traveling Westbound on Tremont Street and could not see well due to a sun glare. Operator of MV1 further stated when she approached the intersection of Tremont Street at Hibbard Road she struck a pedestrian riding a bicycle. I was able to speak with two witnesses on scene who were friends with Ms. Shokri. Both were approximately 10 feet behind her in the crosswalk before she was struck by MV1. Witness statements are documented in this report. Both parents responded on scene and were entered into this report as non-motorists. Both followed medics to Newton Wellesley Hospital. MV1 sustained moderate front end damage. Pedestrian involved suffered minor injuries and was transported to Newton Wellesley Hospital. Operator of MV1 was evaluated on scene and signed a patient refusal. Due to the bicycle being moved prior to my arrival, pictures were not taken on scene. The bicycle was left off the roadway for her friends to pick up. Ms. Skokri was wearing a pink shirt, black shorts and was riding a black bicycle. Ms. Shokri was not wearing any protective equipment.

On 06/10/2020 at 1249hrs I responded to the intersection of Fairway Drive and Watertown Street for a two car motor vehicle crash involving a city of Newton Fire Department vehicle. Vehicle 1 described as a 2009 Ford Taurus Sedan (MA Municipal Fire Reg 64260) was backing southbound on Fairway Drive when it struck Vehicle 2 which was unoccupied and parked in the southbound lane of Fairway Drive. Due to the force of the impact Vehicle 1 sustained minor damage to the drivers side rear quarter panel and Vehicle 2 sustained minor damage to the drivers side rear quarter panel. The operator of Vehicle 1 stated he was backing up due to a large commercial motor vehicle trailer which was parked and obstructing traffic in the northbound lane of Fairway Drive. No injuries were reported on scene. No vehicle tows were required. Pictures were taken and submitted to the NPD IT Bureau Officer Marini N537 responded for the commerical motor vehicle trailer.

On 06/10/2020 at approximately 16:30 hours, Sgt. Lee (Unit 531) and I (Unit 523) responded to Waverley Avenue and Sargent Street for a two car motor vehicle accident that involved one vehicle rolling over and a possible head injury to that driver. The accident occurred at 16:11 hours on this date. Both streets are public ways and maintained by the City of Newton. On arrival, I observed a black Hyundai Elantra (MV1) bearing MA Reg.# 1DHN15 resting in the middle of the intersection facing eastbound on Sargent Street heading towards Kenrick Street. The vehicle had heavy front end damage. I then observed a gray Honda CRV (MV2) bearing MA Reg.# 8DZB10 rolled over resting on it's hood facing in a northeasterly direction in the southbound travel lane of Waverley Avenue just past the intersection. I observed the point of impact to be on the passenger side rear wheel area due to the damage which included a large dent above the tire well. Digital photos were taken of the scene and intersection. At this point the operator, Jose Valderrama (MA DL# S27590058), of MV2 was already transported to Beth Israel Hospital in Boston, MA for a possible head injury. The operator was also not aware of what occurred at the time by the responding officers. It was also determined by the initial responding officers (Sgt. Rooney, Ofc. Pohlman & Ofc. Anderson) that Mr. Valderrama driver's license was suspended in Massachusetts by NPD Dispatch. Next, I spoke to the operator of MV1 identified as Cora Lyden (ME DL# 8625330). Lyden states that she was traveling straight ahead (EB) on Sargent Street attempting to go straight through the intersection on to Kenrick Street. She says as she approached the intersection the signal light changed to yellow and she proceeded through when she suddenly saw MV2 coming at her. She then jammed on her brakes and then made contact with the other vehicle. She says that the other driver must have been speeding and went through the red light. Next, Sgt. Lee and I responded to Beth Israel Hospital for a follow up on Mr. Valderrama. The hospital staff stated that there were no signs of a head injury and that he was now coherent. He sustained road rash on his left arm and shoulder area due to the seatbelt and front air bag deployment. We then spoke to him and he stated that he was traveling straight ahead (SB) on Waverley Avenue at about 30 to 35 miles per hour. He says that his signal light was green when he attempted to go through the intersection and another motor vehicle made contact with him. After that, he does not recall what occurred. I asked him if he saw the other vehicle approaching and he stated that he did not see it. After at the station, I followed up with Dispatch and they stated that there was two people that called in to report the accident that they attempted to call back to see if they witnessed the event. They had since not returned their call. I then called both numbers and both parties stated that they were only notifying us and did not observe the accident occur. Due to conflicting statements by both operators and no witnesses on scene, I am unable to determine fault for the accident. Mr. Valderrama will be issued MA Uniform Citation #T2080466 for Ch. 90/S23/E Operating a Motor Vehicle after Suspension of License. Tody's towing responded and towed both vehicles. M/V inventories completed by me.

On Wednesday 6/17/2020 at approx 2116hrs, while assigned to N-494, I responded to the intersection of Walnut St and Comm Ave for MVA involving a bicyclist. The bicyclist, Aziz Boltayev, said he was traveling NB on Walnut St and attempted to cross Comm Ave. He stated that he was in the middle of the intersection when Comm Ave' light signal turned green. Contact was made to MV1s driver side front panel. Boltayev was transported to Beth Israel with serious injuries to his legs. Boltayev was not wearing a helmet nor wearing reflective clothing. His bicycle, a blue Schwinn, had no front head lamp attached in violation of MGL c85 s11B. A citation was issued and mailed to Boltayev. He was wearing a light colored shirt and shorts. Operator of MV1 states that he was on Comm Ave heading WB and stopped at a red light at the intersection of Walnut St. He states that he had a green signal when attempting to cross the intersection and did not see the bicyclist prior to the accident. Pictures were taken and submitted to IT. The Schwinn bike was placed in property and evidence for safekeeping.

On Thursday July 16, 2020 at approximately 1714 hours I was dispatched to 118 Homer St for a past motor vehicle accident (MVA). Newton Dispatch advised the MVA took place at Commonwealth Ave at Washington St at approximately 1400 hours. I spoke with MV1 operator who stated she was stopped at the red light on Commonwealth Ave (west bound) at Washington St at approximately 1400 hours. Both streets are public ways. MV1 operator stated she was rear-ended by MV2. MV1 operator said she did not sustain any injuries at this time, nor did her MV sustain any damage. MV1 operator stated she and MV2 then pulled over on the other side of Commonwealth Ave and spoke with MV2 operator. MV1 operator said MV2 operator was an Asian male, approximately 45-50 years old, with glasses and dark hair. MV1 operator said she tried to exchange information with MV2 operator but that he did not address the issue and then left. MV1 operator said MV2 sustained minor hood damage due to MV1's hitch. MV1 operator said she was able to take a photo of MV2 registration, which she forwarded to my email. MV1 owner stated he was not going to notify his insurance due to his MV not sustaining damage. Regardless, I advised him the procedure if he decided differently. MV1 owner was satisfied with an accident report.

On Monday, July 27, 2020, I responded to the area of Grant Avenue and Marshall Street, Newton for a report of a motor vehicle crash involving a moped. The weather at the time of the accident was clear and sunny. The road surface was dry. Grant Avenue and Marshall Street are both public ways maintained by the City of Newton. The operator of MV1, Scott Dickerson (S63939963), stated he was operating his 2013 Nissan Altima ( MA: 3JT845) North on Grant Avenue towards Marshall Street. Dickerson stated he observed a party on a moped in front of him stopped on the right side of the travel lane. Dickerson stated he believed this party had pulled over to the side of the road. Dickerson stated he proceeded to drive past this party when the operator of the moped attempted to take a left turn onto Marshall Street. Dickerson stated the front of his vehicle made contact with the moped. I observed damage to the front end of MV1. The operator of MV2, Jerry Bertram (SA2730710), stated he was operating his 2019 YNGF Bintelli 49CC Moped North on Grant Avenue towards Marshall Street. Bertram stated he was stopped in the roadway waiting for traffic in the Southbound lane of Grant Avenue to clear so he could take a left turn onto Marshall Street. Bertram stated as he made his turn MV1 crashed into him. Bertram fell to the roadway and sustained an injury to his left leg/foot. Bertram was wearing a helmet at the time of the crash. Bertram signed a patient refusal with Newton Medics. Bertram has a learner permit to operate the moped. Bertram's moped was removed from the roadway by Tody's Towing. A witness to the crash, Michael Rothstein, stated he observed the crash right at impact and observed the moped fall to the roadway. Rothstein stated he pulled over to check on the operator of the moped. Rothstein provided a ride for Bertram after the crash. Photos were taken by Officer Schlegel and submitted to the IT Bureau.

Vehicle 1 was stopped in traffic on Commonwealth Ave at the intersection of Washington St. Both Commonwealth Ave and Washington St are public ways in the City of Newton. Vehicle 1 was located in the left turn lane of the westbound side of Commonwealth Ave. Vehicle 2 was located westbound side of Commonwealth Ave in the left turn lane. Vehicle 2 was located directly behind Vehicle 1. Vehicle 2 then moved forward colliding with the rear of Vehicle 1. Vehicle 2 then drove around Vehicle 1 and made a left hand turn on to Washington St from the center lane of Commonwealth Ave. Vehicle 2 left the area without leaving any identifying information. As a result of the impact Vehicle 1 sustained damage to the rear bumper. Since Vehicle 2 left the scene the damage to Vehicle 2 is unknown. No injuries were reported on scene, no transports were required. No tows were required. The operator of Vehicle 1 identified as Borris Partensky stated that he was stopped in traffic in the left turn only lane of Commonwealth Ave westbound awaiting the red light. Partensky stated that he was then rear ended by Vehicle 2. Partensky stated that he then got out of this vehicle and signaled to the operator of Vehicle 2 to stop. Partensky stated that Vehicle 2 did not stop, quickly drove around Vehicle 1 and made a left hand turn from the center lane of Commonwealth Ave on to Washington St. Partensky described the operator of Vehicle 2 as a white male in his early 20's with dark hair. Partensky described Vehicle 2 as a newer model Chevrolet Silverado Pickup color white with a paper temporary license plate. Partensky states that the license plate may have started with the numbers "33" and ended with "IT". The state of the license plate is unknown. Partensky stated that his vehicle is a Tesla which has the ability to take photographs. Partensky states he will download the images and send them to me. I also spoke to witness Deb Finamore. Finamore states that she is a city of Newton employee and at the time of the crash was operating her personal vehicle (Ford Escape color Black).Finamore also stated that prior to the crash between Vehicle 1 and Vehicle 2 she observed Vehicle 2 tailgating her vehicle as she was traveling westbound on Commonwealth Ave. Finamore stated that Vehicle 2 tried to pass her several times on Commonwealth Ave and eventually made a left turn off of Commonwealth Ave on to Temple St. Finamore states that she was stopped in traffic at the intersection of Commonwealth Ave and Washington St and was located directly in front of Vehicle 1. Finamore states that she then observed Vehicle 2 located behind Vehicle 1. Finamore states that she that when she got a green signal she made a left turn onto Washington St and heard the impact of the collision between Vehicle 1 and Vehicle 2. Finamore states that she then observed in her rear view mirror Vehicle 2 drive around Vehicle 1 and make a left turn southbound onto Washington St. Finamore states that she attempted to get the registration information of Vehicle 2 however Vehicle 2 continued southbound on Washington St at a high rate of speed. Finamore states that she then followed Vehicle 2 at a safe distance and observed Vehicle 2 commit a red light violation at the intersection of Washington St and the entrance of Newton Wellesley Hospital (2014 Washington St). Finamore states that Vehicle 2 went around stopped traffic at the intersection by entering the left turn only lane to pass stopped traffic to the right. Finamore states that she then observed Vehicle 2 commit another red light violation at the intersection of Beacon St and Washington St. Finamore states that Vehicle 2 entered the left turn only lane at the intersection to pass stopped traffic to the right. Finamore states that she then lost sight of Vehicle 2 as it continued southbound on Washington St. Finamore described Vehicle 2 as a newer model Chevrolet pickup truck color white. Finamore believes that Vehicle 2 had a paper temporary plate, unknown registration number or state of registration. Finamore described the operator of Vehicle 2 as a young white male in his 20's. On 08/03/2020 I was able to view photo's of Vehicle 2 taken by witness Deb Finamore. The photo's revealed Vehicle 2's rear registration plate as 333991T. An NCIC check revealed that this registration belongs to a 2019 Chevrolet Silverado color white out of Maryland. The Maryland RMV also lists the vehicle registration as stolen. No further information was provided by the Maryland RMV. In the photo's Vehicle 2 also appears to be a 2019 Chevrolet Silverado color white. At this time I am unable to identify the operator of Vehicle 2. I called and notified the owner of Vehicle 1 Borris Partensky of my findings. Partensky will be providing this information to his insurance company.

On Saturday, August 1, 2020 at approximately 1810 hours I responded to a single motor vehicle crash. Upon arrival I saw the vehicle completely off the roadway and in the middle of a grassed area next to a large tree. The operator of MV1 stated he was driving westbound on Kenrick St and attempted to take a left turn on Waverly Ave when he lost control of his car and drove over the curb and hit part of the tree. EMS arrived on scene and operator of MV1 signed a refusal. There was major damage to the front of the car making it inoperable. The vehicle was towed by Todys. Todys had to cut branches of the city tree to get the vehicle out. Pictures of the vehicle and tree have been submitted to IT.

On 08/03/2020, while assigned to West Newton Square, I, Officer Conary, took a report of a past mva, car vs bicycle. No police officer was on scene at the time of the accident. Both parties wanted the incident documented. The operator of the bicycle, JIANG, explained to me that he was pedaling Eastbound on Washington Street, crossing over Adam Street, in the crosswalk, when he was struck by MV1. JIANG stated he tried to move out of the way but didn't have time. The bicycle went under the car. Operator of MV1 explained to me that he was traveling Westbound on Washington Street and signaled to take a right turn going Northbound on Adam Street. He said he had a green light and proceeded to take the right turn when he hit the cyclist. There is a regular green signal, no right turn green signal. There was minor damage to the front of MV1 (seen from pictures). JIANG stated that his bike is damaged. All parties did not receive medical attention at this time. JIANG stated that his right shoulder is sore.

On Monday, August 10th 2020 at 0459 hours, I responded to the intersection of Brookline St at Vine St for a one vehicle accident into a City Of Newton Tree. The vehicle was a Black Honda Accord Mass Reg # 7DZ958. Upon arrival, Samuel Perez was outside the vehicle and no operation of the vehicle was witnessed. There was no drugs or alcohol inside the vehicle. The vehicle appeared to have been operated heading west on Vine St. The vehicle continued through the stop sign at Vine St straight into Brookline St where the vehicle collided into a City of Newton Tree. Pictures were taken and submitted to IT. The airbag was deployed. Perez complained of chest and head pain. Perez was taken by Ambulance to Newton Wellesley Hospital. There was extensive damage to the front of the vehicle. Tody's Towed the vehicle. A Towed Motor Vehicle Inventory form was completed. Perez was mailed Mass Uniform Citation #T1273442 in the amount of $ 105 for M.G.L Ch. 89/9 Failure to Stop at Stop Sign.

The operator of MV1 states that he was driving eastbound on Brookline street when another motor vehicle entered his lane of traffic head on causing him to swerve to the right and hit two city trees. No other vehicle remained on scene. MV1 sustained major damage and was towed from the scene by Todys. On 08/12/20 at approximately 13:53 I responded to the intersection of Charlemont St and Roland St for a truck that had taken down some wires. Upon arrival I saw a Harvey Building truck in the middle of the intersection with power and cable lines resting on its cab. I spoke with the operator/employee Bernard McLain who stated he was traveling on Charlemont St and attempted to take a right in the middle of the road as to avoid the low hanging wires. As he took the right, McLain realized he had started to pull on the wires and immediately stopped. Verizon pole #602/10 along with its wires were pulled down and the pole is presently leaning. Verizon and Eversource were notified to respond. At approximately 15:30 a Newton Police Detail relieved me to take over the scene. Photos of the scene were taken and emailed to our I.T. Bureau for attachment to this report. There was no apparent damage to the truck.

On August 13th, 2020, while working in marked unit N526, I responded to a 2 car MVA involving pedestrians at the intersection of Beacon Street and Chestnut Street, Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street and Chestnut Street are both public ways maintained by the City of Newton. The operator of MV1, Maria Dinapoli (S92626906), stated she was operating her 2018 Toyota Corolla ( MA REG: 1KNW71) heading west on Beacon Street and had the green light at the intersection of Chestnut Street. Dinapoli stated she was traveling straight through the intersection when MV2 which was traveling east on Beacon Street at the time cut infront of her vehicle to take a left turn onto Chestnut Street. Dinapoli stated she applied her brakes but was unable to stop in time and made contact with MV2. The impact of the crash caused MV1 to drive over the curb and onto the sidewalk where it made light contact with a baby stroller with a 1 year old inside. MV1 also struck and damaged a Newton Fire Department box that was located on the sidewalk. The baby stroller was being operated by the mother identified as Anna Gilman (S11328154). The pedestrians reported no injuries and signed patient refusals with Medic 2. MV1 sustained moderate front end damage and was towed from the scene by Todys. Dinapoli also reported no injuries and signed a patient refusal with Medic 2. The operator of MV2, Leena Dolabany (S12410136), stated she was operating her 2018 Jeep Grand Cherokee (MA REG: 1TCZ14) heading east on Beacon Street and had the green light at the intersection of Chestnut Street. Dolabany stated she was in a line of cars in traffic waiting to take the left turn onto Chestnut Street. Dolabany stated as the cars infront of her continued through the intersection she took a left onto Chestnut Street and as she was already into the turn she was struck by MV1. MV2 sustained moderate damage to the passenger side panel. Dolabany reported no injuries and signed a patient refusal by Medic 2. A query of all vehicles and parties involved through dispatch determined that MV2 was revoked for insurance. After speaking with the operator I was informed that MV2 had recently switched insurance agencies and Dolabany was able to provide paperwork from Geico Insurance stating the vehicle was insured. Due to the registry not displaying a valid registration, MV2 was towed from the scene by Todys. I advised Dolabany to contact her insurance company and advise them to update their information with the registry so she can retrieve the vehicle. Photographs of the scene and damage to city property were taken by Officer Wilson and submitted to the IT Bureau. On August 14th, 2020 I spoke with the operator of MV2, Leena Dolabany and her father George Dolabany in the front lobby of the police station. They provided paperwork from Geico Insurance that stated the vehicle was infact insured at the time of the accident and that there was an error made by the Registry of Motor Vehicles. Sgt. Devine and I advised them that we have to use the Registry's database for our investigation and because it still states the vehicle is revoked for insurance we can not release the license plates. Sgt. Devine advised them they could get another tow company to come and take custody of the vehicle but the plates will be seized until the registry shows an active status on the registration. I issued Mass uniform citation T2017328 in hand to Leena and advised her she will be summoned for a hearing for the following violations; Ch 90 section 34J Operating MV without insurance and Ch 90 section 9 Unregistered MV.

On Tuesday August 25th, 2020 at approximately 0938 while in N526 I responded to Gordon Rd and Paulson Rd for a City of Newton vehicle that took down Verizon wires. Gordon Rd and Paulson Rd are both owned and maintained by the City of Newton. At the time of the accident the weather was clear and.. sunny and the road surface was dry. On scene was Officer Gabriel who reported that the operator of the 2018 Case Backhoe Tractor identified as Greg Wansiewicz (S81742023) stated that he was traveling on Gordon Rd and turned left onto Paulson Rd when the top of the Backhoe took down low hanging wires. The operator stated the wires were too low and the bucket on the machine was down at the time.The operator reported no injuries and there was no damage to the city vehicle. Pictures of the scene were taken and submitted to the IT Bureau. Verizon was notified and were on scene as well as several DPW vehicles. DPW set up barricades to block off the area until Verizon arrived. The wires that were damaged were from Verizon pole 910/8 to Verizon pole 1002/2.

On Friday, September 18, 2020, at 88update88approximately 0657 hrs, while assigned to N495, I was dispatched to the area of 1221 Centre St for a report of a MV accident. When I arrived on scene I observed a Motor Cycle (MC) laying on its side in the middle of the street. I observed a party laying on the ground, while medical personnel were tending to him. I also observed a white Honda (MV#1) pulled off to the side of Pelham St, and the operator standing next to his vehicle. Operator of MV#1 stated that as he was attempting to make a left turn off of Centre St, onto Pelham St. Operator of MV#1 stated that while he was in the middle of his turn, he observed a MC driving at a high rate of speed coming towards him. Operator of MV#1 stated that he was not able to get out of the way of the MC before it crashed into him I was not able to speak with the operator of the MC, due to him being transported to Brigham and Womens Hospital. Ofc McSweeney stated that he will be following up with the operator of the MC and will advise me. It should be noted that the operator of the MC appeared to be wearing headphones while operating, picture was taken and submitted. An anonymous party stated to me that he observed the MV traveling at a high rate of speed, heading North on Centre St. The party stated that he observed the MC weaving in and out of traffic. Multiple pictures were taken of MV#1 and MC and were submitted to IT to be attached to this report. Operator of MC was transported to Brigham and Womens. Operator of MV#1 was cleared with a patient refusal. Both vehicles were towed by Todys. **UPDATE** I spoke with the father of Scott Mahon, Keith Mahon. Keith stated that his son is still in the hospital, and has received several surgeries. Keith stated that the injuries his son received are a Open Tibia/Fibula fracture of the right leg, and a concussion. Keith stated that Scott has no recollection of the incident, as far as to not remembering leaving the house that morning.
