Operator of vehicle one stated that while driving east bound on Crafts St at Albemarle Rd vehicle two darted out into the path of his vehicle from the south bound lane of Albemarle Rd. Operator of vehicle one stated that he attempted to avoid a crash with vehicle two by applying his vehicle's brakes and swerving to his left. Operator of vehicle one stated that his vehicle's front bumbler made contact with vehicle two's front driver's side fender and bumper. Operator of vehicle one stated that he was not injured his vehicle was not towed. Operator of vehicle two stated that he had stopped at the stop sign in the north bound lane of Albemarle Rd at Crafts St. Operator of vehicle two stated that he saw vehicle one driving east bound on Crafts St and he pulled out into the east bound lane of Crafts St in font of vehicle one believing that he had enough time to cross over Crafts St . Operator of vehicle two stated that vehicle one's front bumper struck his vehicle's front driver's side. Operator of vehicle two stated that he was not injured and his vehicle was not towed. Passenger in vehicle two Zbigniew Saletnik stated that he was not injured and had little recollection of the crash. All Streets are public ways in the city of Newton. I issued operator of vehicle two MA Citation T0340520 and cited him for MLG 89/8 , failure to yield. I took photos of the vehicles and crash area, the disk was downloaded by the NPD's IT bureau.

The operator of MV#1 stated that she was waiting to exit NWH at the east exit/entrance. When her light turned green she proceeded to enter Washington St and her MV was struck by MV#2, who was traveling south on Washington St. The operator of MV#2 stated that she was traveling south on Washington St approaching NWH and had a green light to proceed thru the intersection at the east exit/entrance. An independent witness stated that she was in a MV directly behind MV#1 at the east exit when she observed their traffic light turn green and both her and MV#1 proceeded into the intersection. The witness then observed MV#2 traveling south on Washington St enter the intersection crashing into MV#1 while their light was still green. Operator #2 cited for a red light violation.

The operator of Vehicle #1 stated that he stopped, prior to proceeding through the Stop Sign Southbound at Lewis St, and Newtonville Ave prior to colliding with the bicyclist. The bicyclist stated that he was travelling Westbound on Newtonville Ave prior to colliding with Vehicle #1. Vehicle #1 was cited for a violation of M.G.L. Ch.89/S.9-Stop Sign. It should be noted that the bicyclist was wearing a helmet, and a rear flasher, however wore no other bright, or reflective clothing. All parties declined medical attention. Pictures of the scene were taken by me, and placed in the IT box.

On Wednesday, February 27, 2019 while assigned to Traffic unit N525, I responded to the area of 843 Beacon Street, Newton for a report of a 2 car motor vehicle accident. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1, Omer Chaudhry (NY: 122909834). Chaudhry stated he as operating his 2015 blue Mazda 3 (NY Reg: GVJ1096) Eastbound on Beacon Street towards Centre Street. Chaudhry stated he observed MV2 exiting a parking spot on his right. As MV2 attempted to merge into traffic it's front driver side crashed into the passenger side of his vehicle. I observed damage along the passenger side of Chaudhry's vehicle from the rear passenger tire to the front passenger bumper area. Chaudhry reported no injuries at this time. I spoke with the operator of MV2, Elizabeth Libert (S75526057). Libert stated she was attempting to pulled out of a marked parking spot (E) on Beacon Street when the front driver side of her 2017 gray Mercedes E400 (MA: 1YT269) crashed into the passenger side of MV1. I observed damage to the front driver side tire area of MV2. Libert reported no injuries at this time. Libert's vehicle was removed from the roadway by Tody's towing. Libert was cited with Massachusetts Uniform Citation T1269154 for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care).

On 3/5/19, I responded to Crafts St at Albemarle Rd for a motor vehicle accident. The operator of MV1 (NJ Reg: K89JJU) stated that she was travelling eastbound on Crafts St when MV2 suddenly attempted to drive across the street in front of her. MV1 stated that she attempted to brake but could not do so in time. The operator of MV2 (NH Reg: 4253062) stated that she was stopped at the stop sign on Albemarle Rd when she looking right and saw MV1 driving down Crafts St. MV1 stated that she thought she had enough time to make it across the street, but did not, resulting in the accident. Both parties declined medical attention. MV1 sustained moderate front end damage. MV2 sustained moderate passengers side damage. Neither vehicle required a tow. MV2 was issues MA Uniform Citation T1272249, c89/9.

Operator of MV1 states she was driving Northbound on Centre St. She states she was in the right (straight ) lane, going the speed limit. She states MV2 attempted to turn left in front of her, and she was unable to avoid them. She stated MV2 struck her vehicle on the left front side, sending her off the road and into the traffic signal on the northeast side of the roadway. Operator of MV2 states he was driving southbound on Centre St, attempting to turn left onto Walnut St. He stated he did not see MV1 until the least second, because of the cars in the opposite turn lane. He stated he attempted to stop, but believed MV1 was speeding and he did not have time. Witness 1 was directly behind MV2. He stated it looked like MV2 was attempting to turn left in front of him. He stated MV1 did not appear to be speeding, and that MV2 turned into MV1. He stated he did not think the two cars could see each other until the last second. Witness 2 was in the opposite turn lane. She also stated it appeared MV1 was driving the proper speed, and MV2 turned into MV1. MV1 was towed by Tody's. MV2 was moved off to the side on Walnut St to wait for AAA. 2 parties from MV2 were transported to NWH with minor injuries. Operator of MV2 was given in hand MA citation# T1441667 for 89/8, Fail to yield while turning left. A traffic signal was taken down, and Dagle electric was notified to respond. All other lights were functioning properly. 4 photos were taken of the damage.

Operator of MV1 states she was traveling eastbound on Washington Street approaching the intersection at Walnut Street. Operator of MV1 states the traffic light was green and when she entered the intersection she was struck by MV2 who was taking a left turn from Washington Street westbound onto Walnut Street southbound. MV1 sustained minor damage to the front right bumper. Operator of MV2 states she was traveling westbound on Washington Street approaching the intersection at Walnut Street. Operator of MV2 states she was in the left turn/straight traffic lane when she attempted to take a left turn onto Walnut Street traveling southbound when MV1 struck MV2 in the right rear corner panel. Operator of MV2 states she didn't have the green arrow light, yet had the steady green traffic light. MV2 sustained damage to the right rear corner panel/tire and needed to be towed. It should be noted when the traffic light turns green facing westbound on Washington Street, it gives drivers in the left turn/straight traffic lane the opportunity to take a left onto Walnut Street or travel straight on Washington Street before traffic can flow on Washington Street eastbound. This is displayed by a green arrow light and steady green light. Due to my investigation, I cited the operator of MV2 in hand Massachusetts Uniform Citation #T1441446 for MGL Ch. 90 Sec. 14 Fail to yield right of way when turning

Narrative:
On 03/07/19 at 08:29 hours, I responded to a two car motor vehicle accident at Needham & Oak Streets. The operator of M/V # 1 states she blacked out upon impact of the accident. She was immediately transported to Newton Wellesley Hospital by Cataldo Ambulance for a possible head injury. After at the hospital, Operator of M/V # 1 states she was traveling straight ahead (WB) on Needham Street at Oak Street with the green light signal in front of her. As she approached the intersection, M/V # 2 suddenly attempted a left turn from the opposite side of the road and she was not able to stop in time. M/V # 1 sustained passenger front end damage and the vehicle was towed by Tody's as the operator was transported to the hospital. Operator of M/V # 2 states she was in the left turn lane on Needham Street facing eastbound attempting to take a left turn on to Oak Street. She states that she thought it was clear to proceed and M/V # 1 came out of no where and struck her on her rear passenger side. At first, the operator told me the signal light was green and then after stated that the light was changing to red when she was proceeding through. The vehicle sustained rear passenger side rear wheel and axel damage and it was towed by Tody's. Based on both operator statements, it appears that the operator of M/V # 2 did not yield the right of way and attempted to turn left when it was not clear to proceed. The operator was issued and mailed MA Uniform Citation # T1441703 for Ch. 89/8.

Operator 1 states she was traveling Eastbound on Hammond St when vehicle 2 pulled out of Hammondswood Rd and struck vehicle 1. Operator 2 states she stopped at the stop sign on Hammondswood Rd. Operator two made sure both ways were clear and then pulled out of Hammondswood. Vehicle 1 was traveling at a high rate of speed and struck vehicle 2. Both vehicles sustained moderate damage and vehicle 2 was towed by Tody's. Operator 1 complained of back pain; but, refused medical attention. Operator 2 was not injured. Operator 2's license was suspended and she was issued citation #T0646633 for 89/9 failure to comply stop sign, 90/23/D operating on a suspended license, and 90/23 Unregistered M/V.

On Wednesday, April 10th 2019, at approximately 5:31pm, I, Officer Brooks, responded to Crafts St street at Albemarle road, for a report of a 2 car MVA with injuries. Upon my arrival I spoke with the operator of MV1 (MA REG 61MS57), who stated he was traveling southbound on Albemarle road. He stated he stopped at the stop sign at the intersection with Crafts street, and then proceeded over Crafts street down Albemarle road. The operator stated he was struck by MV2 as he was crossing Crafts street. MV1 sustained heavy damage to the passenger side front end and wheel well area. The operator of MV1 was not injured. I then spoke with the operator of MV2(MA REG 11809), who stated she was traveling eastbound on Crafts street, when MV1 pulled out in front of her. She was not able to stop and her vehicle struck MV1 in the middle of the intersection. MV2 had damage to the front end. The operator of MV2 was complaining of pain in her head from striking the steering wheel. A witness to the accident stated he was stopped behind MV1 at the intersection of Albemarle road and Crafts street. He stated that MV1 accelerated across the intersection without yielding to MV2, and that there was no time for MV2 to stop safely. Both vehicles were towed due to the damage. The operator of MV1 was issued in hand MA Uniform Citation T1442386 for 89/9 Failure to safely proceed after stopping. The operator of MV2 was transported by Cataldo to NWH.

The owner of MV#1 states that he returned to his unoccupied MV to find a note from a witness stating they observed another MV strike his MV while they were attempting to park in the parking spot to the left of MV#1. Witness identified MV#2 as Ma Reg 1705XY, a Honda CRV. Witness states he observed an older W/M operating Ma. Reg. 1705XY (MV#2) attempt to park in a parking spot while striking MV#1 and then quickly exit the spot. Witness feels confident operator of MV#2 knew he struck MV#1. I then spoke with operator/owner of MV#2 who admitted he was operating MV#2 during minor crash, but didn't think it was significant so he left the area. Operator #2 cited for leaving the scene of a MV crash.

On Wednesday, May 29, 2019 while assigned to Traffic unit N525, I responded to the area of Centre Street and Washington Street, Newton for a report of a motor vehicle crash involving a school bus. The weather at the time of the crash was overcast and the road surface was dry. Centre Street and Washington Street are both public ways maintained by the City of Newton. The operator of MV1, Norma Cedeno (S60008138), stated she was operating a 2016 Dodge Caravan (MA SPN: 104893) around the Washington Street/Centre Street rotary. Cedeno stated as she was traveling in the left turn lane on Centre Street, she attempted to travel straight (N) through the intersection at Washington Street towards 255 Washington Street. Cedeno stated the front passenger side of her vehicle crashed into the driver side of MV2. MV2 was attempted to continue left around the rotary and was located on the passenger side of her vehicle. Cedeno reported no injuries. I observed minor damage to the front passenger side tire area of MV1. Cendeno's passenger, Cristina Velez (S04276767), reported not injuries. The operator of MV2, Mary Sanker (S62890912), stated she was operating her 2016 BMW 328XI around the Washington Street/Centre Street rotary. Sanker stated as she was continuing through the rotary in her lane, the vehicle traveling in the lane to her left (MV2) entered her travel lane in an attempt to travel straight towards 255 Washington Street and crashed into the driver side of her vehicle. I observed moderate damage to the front and rear driver side doors of MV1. Sanker reported no injuries. The operator of MV1 was cited for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care)

On Thursday, June 6, 2019 while assigned to Traffic unit N525, I responded to the intersection of Waverly Avenue and Franklin Street for a report of an accident involving a City of Newton school bus. The weather at the time of the accident was clear and sunny. The road surface was dry. Waverly Avenue and Franklin Street are both public ways maintained by the City of Newton. The operator of MV1, Dorothy Wensink (S40227061), stated she was operating her 2010 Toyota Prius (MA: 538EV6) Eastbound on Franklin Street towards Waverly Avenue. Wensink stated she came to a stop at the stop sign on Franklin Street (E) at Waverly Avenue. Wensink stated as she began to travel through the intersection, a school bus traveling on Waverly Avenue (S) crashed into the rear driver side door area of her vehicle. Wensink reported no injuries on scene. I observed moderate damage to the rear driver side area of MV1. Wensink was able to driver her vehicle away from the scene. MV2 was a City of Newton contracted yellow school bus (MA BUN: 39173) transporting students attending the Ward Elementary School. The bus was returning to the Ward School from a field trip. The operator of MV2, Antonio Goncalves (S54275856), stated he was operating the school bus Southbound on Waverly Avenue towards Franklin Street. Goncalves stated MV1 abruptly accelerated across the intersection from Franklin Street. Goncalves stated he was unable to avoid making contact with MV1 and crashed into the rear driver side of the vehicle. I observed minor damage to the front middle and passenger side bumper area of MV2. The following Ward Elementary School student, teachers, and parents were traveling on the school bus at the time of the accident and reported no injuries. Students: Avishai Arbili, Eleanor Chen, Maeve Donovan, Esat Efendigil, Mark Elentuck, Julian Ismay, Rhea Khanna, Griffin Kirby, Ori Layani, Priscilla Le-Ngo, Callum McLaughlin, Carolina Muniz, Emily Ney, Hrithik Ponduru, Levi Rosenberg Van Gameren, Zachary Schwarzberg, Ava Smoller, Matthew Somers, Lillian Welford, William Balerna, Alexandra Bogdanova, Randi Green, Christopher Iatrou Johnson, Charlotte Nichols, Henry Pozen, Sofia Shadah Alcalde, and Zoey Steinberg. Teachers: Katherine Goldman and Lauren Ludman. Parents: Enrique Shadah, Brian Rosenberg and Xinxin Wang. The Principal of the Ward Elementary School, Becca Brogadir, responded to the crash scene and signed a patient refusal for all of the students with Newton Medics. The students, teachers, and parents, then walked back to the Ward Elementary School with Principal Brogadir. The school bus was able to drive away from the scene. Once the students arrived at the Ward Elementary School, the nursing staff there evaluated each child before they could return to class. The school staff stated they would make notifications to the parents of the children involved in the accident. After speaking with all parties involved, the operator of MV1 was cited in hand for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Pictures were taken of both vehicles and submitted to the IT Bureau.

The OP. of MV#1 states he was travelling eastbound on Beacon Street. As he was entering the intersection of Chestnut Street, MV#2 on the oncoming lane suddenly made a left turn in front of his path causing a collision. He pulled over and couldn't find MV#2 anywhere around. He describes MV#2 as a black pick-up truck. The OP. of MV#1 is a dog walker and there were 6 dogs inside the vehicle during the accident. He states the dogs appear fine and the collision wasn't a large impact. Witness #1 states he was the first car stopped on Chestnut Street waiting for the traffic light to turn green. He witnessed MV#2 cut in front of MV#1 and cause a collision. MV#2 then continued driving down Chestnut Street southbound and didn't stop. When they were all stopped at a traffic light for Chestnut Street and Woodward Rd., Witness#1 took down the plate of MV#2 as L92280 and described it as a black pick-up truck with possible MA commercial plates. He describes that it has a spare tire on the bed of the truck leaning against the back window. MV#2 then accelerated away when the light turned green and turned into a unknown side street. The owner of the plate given is for Eric P. Rowe out of Norton, MA. His vehicle model does match the description of a black pick-up truck. I gave Norton P.D. a call at approx. 1800 hrs. A short while later, they called back and said the pick-up wasn't home but gave me a cell phone number. I was able to speak with Eric Rowe and he stated he was the OP. of MV#2. He stated he was on Beacon Street westbound making a left towards Chestnut Street southbound. There was a car making a turn in front of him and he saw MV#1 slow down, which is why he made the turn. A minor collision happened so he stopped to look at this truck and saw no damage. He didn't see MV#1 stopped anywhere so he drove away thinking there was no need to exchange paperwork for no damage to his truck. The OP. of MV#2 was advised of the proper procedure and that he will be getting a citation in the mail. An incident report was also generated.

The hood was bent in, the bumper was cracked and hanging, and fluids were leaking from the vehicle's engine. I next observed the damage to the doors on the right side of MV#2. I observed the base of the doors were pressed in, those doors would not open and the tires were grinding against the flared edges of the wheelwells. - There were no injuries reported at the scene, however, both operators were checked by Ambulance 1. Newton Fire responded to scene to contain the Hazmat spill from MV#1. MV#1 was towed from the scene. OpMV#1 was cited (T2015541) for Failure to come to a Complete stop at stop sign. I transported Operator of MV#1 to Tody's towing from the scene.

I observed that the plastic bumper was separated from the frame and several cracks and breaks on the bumper. I further observed damage to the wheel well panel. On Saturday, September 28, 2019, I responded to the intersection of Crafts Street and Albemarle Road, Newton with Sgt. Lee for a report of a motor vehicle crash involving a motorcycle with serious injury to it's operator. The weather at the time of the crash was clear and sunny. The road surface was dry. Crafts Street and Albemarle Road are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jussara Pontes (S92930243). Pontes stated she was operating a 2017 Toyota Sienna (MA: 4GX777) at the time of the crash. Pontes stated she was traveling North on Albemarle Road and came to a stop at the stop sign located at the Crafts Street intersection. Pontes stated she looked both ways and began to inch out into the roadway. Pontes stated she stopped the vehicle again and was assisting a child sitting in the 2nd row of the vehicle. Pontes stated she then turned her attention back to the roadway. Pontes stated she looked to her right and left again and did not see any vehicles in either direction. Pontes stated she proceeded to cross the intersection to continue North on Albemarle Road. Pontes stated the front bumper area of MV1 crashed into the left side of a passing motorcycle. Pontes stated the motorcycle and it's operator fell to the roadway and slid across the street to the side of the road. Pontes stated she pulled her vehicle over onto Albemarle Road. Pontes stated she did not see the motorcycle prior to the crash and was not distracted. The operator of the motorcycle (MV2), Irene Pistoftzian (S06099449) was transported to Brigham and Women's Hospital with a serious left leg injury prior to my arrival. I observed a red 1990 Kawasaki ZR550 (MA MCN: 1V1063) resting on it's right side on the NE sidewalk area of Albemarle Road at Crafts Street with it's front wheel/handlebar facing Crafts Street. The motorcycle had moderate damage to it's left side from the impact of MV1 crashing into it. The right side of the motorcycle had damage from sliding on the roadway and the crashing into a fence on the sidewalk on Crafts Street (W) at Albemarle Road. There was also damamge to the front headlight/right handlebar area. The motorcycle and Toyota Sienna were towed from the scene by Tody's towing at Sgt. Lee's request. Pictures were taken of the scene and submitted to the IT Bureau. I spoke with Pistoftzian at Brigham and Women's Hospital in Boston on Monday, September 30, 2019. Pistoftzian stated she was operating her motorcycle on Crafts Street (W). Pistoftzian stated as she approached the intersection of Crafts Street and Albemarle Road, she observed a mini van to her left out of her peripheral. Pistoftzian stated she remembers being hit on her left side and then laying on the roadway looking up at the sky on the sidewalk with her head facing Albemarle Field. Pistoftzian stated as a result of the crash she has a significant injury to her left leg and also has an injury to her left wrist/arm. Pistoftzian stated she was wearing a helmet at the time of the crash. I spoke with Kymberley Mitchell via telephone who stated she witnessed the crash. Mitchell stated she was traveling behind the motorcycle involved in the crash on Craft Street (W) at North Street. Mitchell stated they were stopped at a red light at this time. Mitchell stated there were no vehicles in front of the motorcycle at the light. Mitchell stated the light turned green and they continued on Crafts Street at a low rate of speed. Mitchell stated she could see ahead at the intersection of Albemarle Street and Crafts Street, traffic was stopped on Crafts Street (E) letting traffic exit Albemarle Road (N). Mitchell stated vehicles were taking left and right turns out of Albemarle Road before the motorcycle reached the intersection. Mitchell stated a mini van was crossing past the double yellow line of Crafts Street to continue straight when it crashed into the left side of the motorcycle. I spoke with Cheryl Wermer via telephone who stated she witnessed the crash. Wermer stated she was traveling on Crafts Street (E) towards Albemarle Road. Wermer stated traffic ahead of her was stopped to allow vehicles to turn off of Albemarle Road. Wermer stated she observed a motorcycle traveling towards her on Crafts Street (W). Wermer stated she then saw a vehicle travel straight from Albemarle Road across Crafts Street and crash into the motorcycle. Wermer stated the operator of the motorcycle flew off the bike. Wermer stated the car then came to a stop and the driver got out and stated "Oh my god! I didn't see you!" Wermer stated both vehicles were traveling at a low rate of speed prior to the crash. I spoke with Bill Frabizio via telephone who stated he witnessed the crash. Frabizio stated he was behind the mini van (MV1) on Albemarle Road prior to the crash. Frabizio stated the vehicle in front of him began to cross the intersection as he was monitoring the traffic on to take a right turn onto Crafts Street. Frabizio stated he heard a loud "Boom" and saw a motorcycle slide across the street with its operator. Frabizio stated the motorcycle crashed into a fence post and the operator came off the motorcycle. Frabizio stated he pulled over and assisted operator of the motorcycle. After concluding my investigation, the operator of MV1, Jussara Pontes, was mailed Massachusetts Uniform Citation T2016424 for Newton City Ordinance Chapter 19, Section 75 (Failure To Use Care).

HE STATES HE WAS TRAVELING E-BOUND ON CRAFT INTENDING TO GO TO THE POST 440 ON CALIFORNIA ST. TO ATTEND AN AA ( ALCOHOLICS ANONYMOUS ) MEETING. HE STATES HE TOOK HIS SON'S MOTORCYCLE WITH THE INTENTION OF SEEING IF THERE WERE ANY REPAIRS NECESSARY BECAUSE HIS SON IS TRYING TO SELL IT. WHILE TRAVELING HE APPROACHED THE INTERSECTION. HE SAW VEHICLE #2 ENTER ONTO CRAFT ST. HE STATES HE COULD NOT DIVERT AND HE WAS UNABLE TO AVOID HITTING HER VEHICLE. OPERATOR HAS BROKEN BONES IN HIS LEFT ARM. CUTS ALONG HIS FACE AND NECK AS WELL AS LOWER BODY PAIN. HE STATED THE HOSPITAL INTENDS ON KEEPING HIM OVER NIGHT FOR OBSERVATION. I ASKED THE DRIVER IF HE WAS AWARE THAT HIS LICENSE TO OPERATE A MOTOR VEHICLE WAS REVOKED. HE STATED HE HAD A CHANCE TO BE REINSTATED BUT CHOSE NOT TO. DRIVER WAS ISSUED MOTOR VEHICLE CITATION ( T1445178, 90/23/C, LICENSE REVOKED AS HTO ). I INFORMED THE OPERATOR THAT I WOULD MAIL HIM THE CRIMINAL APPLICATION. ALL OTHER PARTIES REPORTED NO INJURIES. BOTH VEHICLES WERE TOWED BY TODYS. VEHICLE #1 HAD FRONT END DAMAGE AND FRAME DAMAGE. VEHICLE #2 HAD LEFT SIDE DAMAGE AND A BROKEN AXLE. WEST NEWTON SQUARE ( OFF L. MIKOLEIT ) ARRIVED ON SCENE AND TOOK 10 PICTURES. PICTURES PLACED IN I.T. BOX. I CLEARED THE AREA WITHOUT FURTHER INCIDENT.

Narrative:
On 10/04/19 at 11:49 hours while assigned to Traffic Unit 523, I responded to a motor vehicle/bicyclist accident at Centre Street and Nathan Road. The weather at the time was cloudy. The road surface was dry and Centre Street and Nathan Road are public ways maintained by the City of Newton. Traffic conditions were heavy and Centre Street was congested with vehicles at the time. There is no dedicated bicycle lane in this area but there are bicycle indicators painted on the roadway to make drivers aware of bicyclists traveling. I spoke to the bicyclist involved (Unit 2), Marvin Goldman (S67373495). Goldman was wearing a helmet, gray sweatshirt, gray shorts, and black/orange reflective sneakers. Mr. Goldman stated that he was traveling southbound on Centre Street on the right side of the road on his black Fuji Sagres bicycle. He further says that he was peddling fast at the time while crossing over Nathan Road and that is when he suddenly struck Unit 1 on it's passenger side front quarter panel/door area. He does not recall any other details of the incident along with Unit 1 attempting to turn left towards him. Goldman sustained scraped up knees and a small laceration below his left eye. He refused medical attention signing a patient refusal with Cataldo Medic One. I observed that his front wheel was bent as a result of the accident. The rest of the bicycle appeared to be intact. I then spoke to the operator of Unit 1, Darlene Martins (S27966454). Ms. Martins spoke with the assistance of Ofc. Calderon who was translating for her. She says that she was traveling northbound on Centre Street and was attempting to turn left on to Nathan Road in order to turn around and travel the other way on Centre Street. She says that a vehicle in the southbound travel lane left space for her to make her left turn. She then proceeded to turn thinking it was clear and that is when the bicyclist suddenly approached and struck her on her passenger side. She also stated that it was hard to see past the congestion of vehicles waiting in the southbound travel lane. I observed scratch marks and a dent to her passenger side front quarter panel area along with minor scuff marks to her front passenger door. The scuff marks appear to be from the bicyclist rubbing against the vehicle. I also observed a dent on the top of the passenger door frame. It is unknown if the damage is the result of the accident or old damage Ofc. Calderon spoke to Rosangela Dias (Passenger in Unit 1), who stated that everything happened so quick but she never saw the bicyclist approaching when Martins attempted to turn left. I then spoke to a witness on scene identified as Matthew Miner. He says that he was two vehicles behind an unknown vehicle in the southbound travel lane of Centre Street. He says that the unknown vehicle left space for Unit 1 to turn left and waved Unit 1 on to turn left. He does not recall what transpired with the bicyclist. As a result of the accident, it was determined that Martins did not yield the right of way to the bicyclist while she was turning left. She also does not have a license to operate in Massachusetts or any other state. She was issued MA Uniform Citation # T2012393 for Ch. 89/8 Failure to yield and Ch. 90/10 Unlicensed Operation of a Motor Vehicle. Digital photos taken by me of the scene.

Operator of vehicle 1 stated he was traveling eastbound on River St and was taking a left turn onto Cherry St and was struck head on with vehicle 2. Operator of vehicle 2 stated she was traveling westbound on River St and as she was entering the intersection of Cherry St the traffic light turned yellow and vehicle 1 which was traveling eastbound made a sudden left turn leaving her no time to stop and the vehicles collided. Vehicle 1 then sped off northbound on Cherry St and returned a minute or two later. I spoke to a witness, Jena Castro-Casbon who stated she saw vehicle 1 traveling northbound on Cherry St at a high rate of speed and drop of a child at 187 Cherry St then turn around and return to the accident scene at a high rate of speed again. 187 Cherry St is 0.4 miles from the intersection of River St. Operator of vehicle 1 stated he had two of his sons friends in the vehicle and he thought it would be best to get one of them home to 187 Cherry St and then come back to exchange information. I followed up with the child and his parents at 187 Cherry St and he was uninjured. I issued the operator of vehicle 1 MA Uniform Citation #T1445381 for 89/8 failure to yield. No one reported any injures and neither vehicle required a tow.

On 12/23/19 at 12:06 hours, I came across a two car motor vehicle accident that already occurred at California Street and Bridge Street. No injuries reported. Both streets are public ways and maintained by the City of Newton. Operator of M/V # 1 states he was traveling straight ahead (EB) on California Street through the intersection at Bridge Street. He states that he had the green light signal and attempted to drive through when he was struck by M/V # 2 on his front bumper on the driver's side. He further says that M/V # 2 was attempting to take a left turn on to Bridge Street at the time of the accident. I observed moderate damage to the driver's side front bumper/quarter panel area but the vehicle was still driveable. Operator of M/V # 2 states through translation by her husband (Huseyn Imamaliyev) she was operating (WB) on California Street and was attempting to take a left turn through the intersection on to Bridge Street. She says that she stopped first and then attempted to turn left and that is when M/V# 1 crashed in to her. I observed minor damage to the middle front bumper/registration plate area. Based on the locations and damage to the vehicles, it appears that the operator of M/V # 2 did not yield the right of way when turning left on to Bridge Street. The operator of M/V # 2 (Gulnar Imamaliyeva MA ID# SA1320134) also could not provide a driver's license to operate a motor vehicle in Massachusetts. A query of her revealed that she only had a Massachusetts identification. She was issued MA Uniform Citation # T2012157 for Ch. 89/8 Fail to Yield Right of Way When Turning Left and Ch. 90/10 Operating a M/V Without a License. Narrative:
Vehicle one, Newton police marked 503 cruiser operated by Officer Hagopian, was stopped behind a stopped unknown vehicle on Watertown Street facing West waiting on a red light. Officer Hagopian stated both vehicles were in the right travel lane. Vehicle one was the second car in line at the red light. Officer Hagopian of vehicle one heard a police broadcast of a victim in cardiac arrest and activated its police emergency lights and siren to respond to the emergency call. The unknown vehicle in front of vehicle one pulled to the right and allowed vehicle one to navigate around it. Officer Hagopian stated he had his emergency lights and siren activated and went around the stopped vehicle that had pulled to the right in front of him. Vehicle one proceeded West toward the intersection at a slow rate of speed and observed the intersection to be clear and then accelerated. Officer Hagopian stated that vehicle two came from Walnut Street, traveling North, and came across the intersection where vehicle one and two collided. Officer Hagopian estimated his cruiser to be traveling at 15-20 mph upon impact with vehicle two. The front end of vehicle one made impact with the passenger side of vehicle two. Vehicle two, BMW operated by Gail Glaser, was traveling North on Walnut Street and was approaching the intersection of Walnut Street and Watertown Street. Operator of vehicle two estimated her vehicle speed to be about 20 mph as her vehicle entered the intersection. Ms. Glaser stated she had a green light. Operator of vehicle two was in the middle travel lane that was a designated lane to go straight. Mrs. Glaser stated she did not see an emergency vehicle but could hear the siren as she approached and then entered the intersection from Walnut Street (going North). She stated she had a clear view of the intersection. Vehicle two then came in contact with vehicle one (going West on Watertown Street) in the intersection. Vehicle two then came to a final rest in the Southbound lane of Walnut Street near the traffic signal posts at the corner. Vehicle two made head on contact with vehicle three upon final rest. Vehicle three, Acura operated by Jeffrey Katz, was the first car in the right travel lane on Walnut Street facing South. Mr. Katz stated he, vehicle three, was stopped at a red light that had just turned to a green light. Mr. Katz stated he heard the siren, stay stopped, and looked towards the direction of the sound and saw a marked cruiser entering the intersection from Watertown Street (West). Mr. Katz remained stopped where he was and then saw vehicles one and two collide in the intersection. Vehicle two then veered into the front of his vehicle head on and contact was made. Mr. Katz stated when he initially saw the cruiser it was already in the intersection with emergency lights and siren activated and estimated the cruiser to be going about 20 mph upon impact. Mr. Katz also indicated that vehicle two was traveling into the intersection from Walnut Street (North) at an estimated 20 mph. Witness Peter Dill was sitting in his parked car on Watertown Street facing East. Mr. Dill was parked in front of his house at 606 Watertown Street. It is the second home in from the intersection on the Eastbound travel side of Watertown Street. Mr. Dill Stated that he was looking at his cell phone when he heard the siren. When he looked up towards the sound of the siren, he saw vehicle one traveling about 5 mph in the intersection with emergency lights and siren activated. He did not know where vehicle one had originated from and just that it was traveling West on Watertown Street. Mr. Dill also saw vehicle two approaching from Walnut Street heading North and was not speeding. Mr. Dill estimated that 3-5 seconds had gone by when he first heard the siren to when the initial impact of vehicles one and two occurred. Witness James Davis-Sanon was driving a large MWRA utility work truck. He was the first car in the right turn only lane on Walnut Street. He stated that Walnut Street had the green light and he stopped before the intersection to allow vehicle one to continue going West on Watertown Street. Mr. Davis-Sanon stated he saw/heard vehicle one activate its emergency lights and siren when vehicle one was stopped behind a car waiting on a red light on Watertown Street. After vehicle one activated the emergency lights and sirens, it went around the car stopped in front of it and proceeded into the intersection at a slow rate of speed that Mr. Davis-Sanon described as a "rolling stop" but could not remember if the cruiser stopped or nearly stopped. Mr. Davis-Sanon stated vehicle one then accelerated to about 15 mph into the intersection. Mr. Davis-Sanon also looked at his driver's door rear view mirror and saw vehicle two traveling North on Walnut Street in the designated straight only lane and then proceed into the intersection. He estimated that vehicle two was traveling at 25 mph upon impact with vehicle one. Witness Joseph Giangregorio was in the front passenger seat of the same MWRA utility work truck. Mr. Giangregorio concurred with Mr. Davis-Sonon witness accounts regarding the location of their truck and vehicle one prior to emergency lights and sirens. Mr. Giangregorio did not witness the rear view mirror account since Mr. Giangregorio did not observe vehicle two in the rear view mirror. Mr. Giangregorio added that vehicle one activated its emergency lights and siren, maneuvered around the stopped car in front of it, slowed to 1-2 mph before the intersection but could not remember if it was stopped and then accelerated to about 15 mph before impact with vehicle two. He estimated that vehicle two was traveling 25 mph upon impact with vehicle one. I canvassed the area of where this incident ocurred for cameras. The only cameras I was able to locate was at 575 Watertown Street which is the second house in from the intersection on the Westbound travel side. Both cameras at this address did not have an angle to capture the intersection. Both cameras are focused on the front door to the home and the homeowner, Jose, confirmed that the angle could not capture the intersection. Operator of vehicle two was cited for failure to yield to an emergency vehicle, MA uniform citation T2012104. Citation was written on February 3, 2020 and sent via mail.

On 02/08/20 I responded to the intersection of Winchester St and Curtis St for a two vehicle crash. M.V. #1 stated she was driving due West on Winchester St. when veh #2 side swiped her in the same direction. Criminal Citation #T2013434 for Ch 90 / Sec 23- Operating after License Suspension (subsequent offense) and Ch 90 / Sec 9- Unregistered M.V. on a public way. Operator of veh #2 has 11 arraignments on his adult B.O.P. with mostly motor vehicle offenses. I completed a towed m.v. form for veh #2, scanned it into this report and placed the citation into the court packet. All roads traveled/mentioned in this report are public ways in the City of Newton as they are serviced and maintained by this city.

On March 16th, 2020 at approximately 09:38 hours while working N491 I responded to a report of two car MV crash at the intersection of Newtonville Ave at Lewis Terrace. The other involved vehicle(#2) was located inside the construction site at #255 Newtonville Ave. I spoke with the operator of the Truck(veh#1) who reported that he was driving W/B on Newtonville Ave when vehicle #2 pulled out into the intersection from north side of Lewis Terr striking the front passenger side of his truck. The impact from the crash caused vehicle #2 off the road, through a chain link fence and into the construction sit to #255 Newtonville Ave before coming to a stop. There were numerous construction worker on the job site but miraculously none of them were struck by vehicle #2 as it crashed through the fence onto the site. I later spoke with the operator of vehicle #2 who was not injured. he stated he was driving S/B on Lewis Terr. and was entering onto Newtonville Ave when he was struck by vehicle #1. Operator couldn't remember if he looked for oncoming traffic prior to entering the intersection. I spoke with the son of Mr Goldfinger(operator #2) who had arrived on scene and he assured me his father would no longer be driving again. I cited Mr Goldfinger for failing to yield/stop(c89/c9) at an intersection. Ticket # t1444075 Todys towed vehicle#2 from scene and there were no reported injuries. Witness stated she was behind vehicle #2 on Lewis Terr.and observed him slowly enter out into the intersection without coming to a complete stop. Pictures taken by Construction foreman. On Thursday 4/30/2020 at aprrox 1859hrs, while assigned to N-494, I responded to the intersection Washington St and Centre St for a 2 car MVA. There I observed MV2 stopped in the far left lane and MV1 stopped in the adjacent lane. Operator of MV1 stated that he was crossing over the Washington St bridge and stayed in the second lane furthest from the left and was headed to the I-90 on ramp. He said that he saw MV2 in his side view mirror try to enter his lane and hit his U haul trailer. Operator of MV2 states that he was coming from Centre St and was attempting to head WB on Washington St. It should be noted that the two left lanes are for the I-90 on ramp only and that his merging lanes had a posted yield sign. On May 17th, 2020 at approximately 14:26 hours while working N491 I responded along with NFD and Medic One to a mv crash at the intersection of Watertown St @ CRAFT St. On my arrival both the Fire Dept. and Medics were assessing the operators of the two involved vehicles. Operator from vehicle #1 a 2014 black Jeep identified as Mark Mccullough was transported from the scene by ambulance to Newton Wellesley due to his injuries. He did give a statement prior to being transported.. He stated he was going N/B on Craft St when vehicle#2 suddenly made a left turn in the intersection towards Watertown St causing the crash. Operator of vehicle #2 reported going S/B on Craft St and was attempting to turn left onto Watertown St when vehicle #1 crashed into him in the intersection. Witness id as Eamen Keane stated he was behind vehicle #1 on Craft St when he observed vehicle #2 turn left in front of vehicle #1 causing the crash. I cited operator #2 for failing to yield at an intersection C89/S8.additionally charged him with operating his mv without a valid license C90/S10. He did possess a Guatemalan license. His vehicle was registered to him with an address of #59 Chester Ave Waltham Ma. e . He was additionally charged with operating his vehicle on a public way without being properly licensed.C90s10.Citation #T2014121. Both vehicles were towed by todys. Both drivers sustained injuries but only operator #1 went to the hospital. Operator #2 refused. All above mentioned streets are public ways and maintained by the city of Newton DPW.

On Monday, August 10th 2020 at 0459 hours, I responded to the intersection of Brookline St at Vine St for a one vehicle accident into a City Of Newton Tree. The vehicle was a Black Honda Accord Mass Reg # 7DZ958. Upon arrival, Samuel Perez was outside the vehicle and no operation of the vehicle was witnessed. There was no drugs or alcohol inside the vehicle. The vehicle appeared to have been operated heading west on Vine St. The vehicle continued through the stop sign at Vine St straight into Brookline St where the vehicle collided into a City of Newton Tree. Pictures were taken and submitted to IT. The airbag was deployed. Perez complained of chest and head pain. Perez was taken by Ambulance to Newton Wellesley Hospital. There was extensive damage to the front of the vehicle. Tody's Towed the vehicle. A Towed Motor Vehicle Inventory form was completed. Perez was mailed Mass Uniform Citation #T1273442 in the amount of $ 105 for M.G.L Ch. 89/9 Failure to Stop at Stop Sign.

Narrative:
On Tuesday, October 6, 2020, I received a call at approximately 2158 hours from Sgt. Devine of the Newton Police Traffic Bureau. Sgt. Devine stated there was a motor vehicle crash with serious injury that had just occurred at the intersection of Crafts Street and Waltham Street in Newton. Sgt. Devine stated that one of the parties involved had suffered life threatening injuries and was transported to Brigham and Women's Hospital. I responded to the scene and conducted an investigation of the crash with the assistance of Sgt. Devine and Captain Boudreau. Upon arrival it was reported to me by officers on scene that a party operating a motorized scooter on Crafts Street (E) had been struck by a 2007 Chrysler Town and Country (MA: 5EE316). The Chrysler was attempting to make a left turn from Crafts Street (W) onto Waltham Street (S). The operator of the motorized scooter, identified as Nadhir Tchikou (DOB: 11/23/1993), had suffered serious injuries to his head and left leg area and was transported to Brigham and Women's Hospital. It was reported to me that Officers Conary, Boyle, and Donovan were the first to arrive at the crash and provided life-saving care to Mr. Tchikou prior to being transported to Brigham and Women's Hospital. Officer Donovan responded to Brigham and Women's to keep us updated on Mr. Tchikou's status. The update provided to me while on scene was that Mr. Tchikou was in critical condition and in surgery. Due to the seriousness of the crash and potential fatal injury, Sgt. Devine contacted the Massachusetts State Police to assist in the investigation. Sgt. Mahoney and Trooper Paul responded as part of the Massachusetts State Police Crash Reconstruction team and conducted a supplemental investigation of the crash (MSP report 2020-CAR-000285). The location where the crash took place is Crafts Street and Waltham Street. Crafts Street and Waltham Street are public ways maintained by the City of Newton. Crafts Street and Waltham Street are municipal roadways that support two way travel for motor vehicles and bicycles with an unprotected median in the middle of both roadways to separate the direction of travel. This intersection was recently re-paved within the last month and has newly painted street markings including crosswalks, dedicated bicycle lanes, and stops lines at the intersections. Crafts Street at this intersection features two travels lanes. I observed the road surface to be dry at the time of my arrival on scene. The north lane in the westbound direction of Crafts Street permits travel straight ahead and also permits motorists/bicyclist to take a right turn onto Harding Street. The south lane in the westbound direction of Crafts Street is a dedicated left turn only lane that coincides with a left turn only arrow on the traffic signal in the middle of the intersection. The opposite side of Crafts Street permits traffic traveling in the eastbound lane to travel straight or take a left turn onto Harding Street from is a single lane travel lane. The area is well lit featuring street lights bright enough to illuminate the intersection for motorist traveling through the area. The weather at the time of the crash appeared to be clear. In the intersection where the crash took place, I observed a large debris field comprised of parts from a 2020 Tao Tao motorized scooter (49CC) and a 2007 Chrysler Town and Country (MA: 5EE316). I observed the motorized scooter (VIN: L9NTCBAG1L1010287) resting on its side facing North in the northbound lane of Crafts Street in the intersection. The motorized scooter has significant damage to its front, right, and left side and was inoperable. The area in the immediate vicinity of the motorized scooter was saturated with gasoline. The rear lights and dash of the motorized scooter were illuminated while I was observing the crash scene. The headlights were not intact due to the impact from the crash and I was unable to determine if they were functioning. The motorized scooter was removed from the roadway by Tody's Towing. The intersection where the crash took place had numerous first responders present immediately after the crash rendering aid to Mr. Tchikou prior to his transport to the hospital. It was reported by officers on scene that some of the items and debris in the intersection had been moved from their initial place of rest after the crash took place. A black MMG helmet was observed resting on the ground outside the debris field from the crash before the westbound left turn only lane of Crafts Street. It is unknown if it was moved from its initial place of rest by a first responder. I observed the strap on the helmet that would be used to secure it to the operator to be in good operable condition. I did not observe it to be defected and the securing feature of the strap was in working order. I did not observe any blood, hair, or other apparent physical evidence inside the helmet that would be consistent with the head injury reported to the operator of the motorized scooter. I observed what appeared to be fresh markings on the front mouth/nose area of the helmet. I observed a white Iphone with a red phone case lying on the roadway a short distance away from the impact area. The Iphone was on and displaying travel directions from the Iphone application Waze. The Iphone and the helmet were seized pending the status of the operator of MV2. A property/evidence form was completed and the Iphone and helmet were secured in property locker number 9. I spoke with the operator of MV1, William Moran (S34946215). Moran stated he was operating his white 2007 Chrysler Town and Country (MA: 5EE316) on Crafts Street (N) as he approached Waltham Street. Moran stated he had a green light and activated his left blinker as he approached the intersection with the intention of taking a left turn onto Waltham Street. Moran stated as he proceeded to take his left turn he saw "orange" and was unable to avoid hitting the object in front of him. Moran stated the object came out of nowhere and he swerved to the left in an attempt to avoid the crash. Moran stated after the collision he accelerated away from the impact area and came to a controlled stop on Waltham Street (S) just past the intersection. Moran stated he exited his vehicle and looked back at the intersection he had just travelled through. Moran stated as he began walking towards the intersection he observed a party laying on the roadway in the intersection. Moran stated the party was not wearing a helmet at this time. Moran also added that he believes the moped involved in the crash did not have its headlights on prior to impact. Moran stayed on scene and cooperated fully with officers during the investigation. A query of Moran's license and registration status through Newton Police Dispatched showed they are both active. I asked Moran if he was using a cell phone, GPS, or any other type of hand held device prior to the crash. Moran stated he was not. I asked Moran if he was distracted by anything either inside the vehicle or something on the roadway prior to the crash. Moran stated he was not. I asked Moran if he was currently impaired in any way that would impede his ability to operate a vehicle safely. Moran stated he was not. I asked Moran if he takes any medications or has a medical condition that would impede his ability to operate a vehicle safely. Moran stated he does not. Moran reported no injuries as a result of the crash. I observed Moran's vehicle at its controlled rest location. The vehicle was parked on Waltham Street (S) in the southbound lane just past the intersection of Crafts Street. The 2007 Chrysler Town and Country (MA: 5EE316) had substantial damage to the front passenger side bumper, hood, fender, windshield, and passenger door area. There was physical evidence of what appeared to be hair, blood, and tissue presumed to be from the operator of MV2 present in the cracked windshield, and on the passenger side door/mirror area. I observed a blue Commonwealth of Massachusetts "R" inspection sticker with a date of 05/28/2020 affixed to the windshield. Moran stated the rejection sticker was a result of an emissions failure. The inspection sticker expires in 2021. I asked Moran if there were any other mechanical issues with the vehicle. Moran stated there were no issues to his knowledge. I observed no objects present inside the vehicle during my investigation that would have potentially impeded the operators ability to see clearly through the windshield. I observed glass debris from the windshield inside the passenger and driver area of the vehicle. There was no air bag deployment inside the vehicle. Subsequent to my investigation, the vehicle was seized pending the status of the operator of MV2. The vehicle was towed from the scene by Tody's towing and secured in the Newton Police Department garage. The vehicle was escorted to the garage by Officer Donovan. A towed motor vehicle form was completed by Officer Donovan and the key for the vehicle was secured in property locker number 9. A property/evidence form was completed. Subsequent to my investigation Moran's black Alcatel cell phone was seized pending the status of the operator of MV2. A property/evidence form was completed and the phone was secured in property locker number 9. Officer Conary stated to me that prior to my arrival on scene she interviewed a witness to the crash and provided me with the following information. Joshua Hamel was operating his vehicle (W) in the right lane of Crafts Street towards Waltham Street. Hamel stated that MV1 (MA: 5EE136) was in the left turn lane on Crafts Street (W) towards Waltham. Hamel stated the scooter was headed eastbound on Crafts Street from Waltham. Hamel stated MV1 began to take a left turn and there was a head on collision with the scooter. Hamel stated he believed the traffic light at the time of the crash was green. On Friday, October 9, 2020, I spoke with Hamel via telephone. Hamel corroberated the statement he gave to Officer Conary. I asked Hamel if he believed the headlights on the motorized scooter were activated at the time of the crash. Hamel stated he believes the headlights on the motorized scooter were on as it approached the intersection but was not postive. On Wednesday, October 7, 2020, I spoke with Mr. Tchikou in his ICU room at Brigham and Women's Hospital. Tchikou's wife, Yami Carrajza, was present in the room while I spoke with him. Tchikou stated he had a significant head injury, injuries to his nasal cavity, his back, and a significant injury to his left leg. I asked Tchikou about his recollection of the crash. Tchikou stated he had just left his friend's home on Hazelhurt Avenue in Newton and was traveling home to Brighton to bring his wife dinner. Tchikou stated he was traveling Eastbound on Crafts Street as he approached the intersection of Crafts Street and Waltham Street. Tchikou stated he had a green light and as he entered the intersection a white vehicle took a turn from the opposite side of the road in front of him. Tchikou stated he does not remember anything after that. I asked Tchikou if he was wearing a helmet at the time of the crash. Tchikou stated he was. I explained to Tchikou that I recovered a white Iphone with a red case at the scene of the crash. Tchikou identified the Iphone as his. I explained that the phone was displaying travel directions when I found it and I asked Tchikou if he was distracted by his phone prior to the crash. Tchikou stated he was not distracted by the phone and the phone was in a holder on the motorized scooter. I asked Tchikou if he remembers if his headlights were activated on his motorized scooter at the time of the crash. Tchikou stated they were on. I asked Tchikou if he could produce paperwork for the motorized scooter to prove he was the owner. Tchikou stated he had recently purchased the scooter and the paperwork for it was at his residence. Tchkou's wife e-mailed me a picture of the Certificate of Origin for the motorized scooter. I asked Tchikou if he could produce a license or permit that would allow him to operate the motorized scooter. Tchikou stated he did not have a Massachusetts license or permit, but had an Algerian license at this residence. I was able to observe a picture of the license on his wife's cell phone. The written text on the license was in Arabic. Tchikou did not have a translation of the license in English and did not possess an International Driver's Permit. As a result of my investigation, Mr. William Moran while operating MV1 (MA: 5EE316) entered the travel lane of MV2 (Tchikou) as he was making a left turn onto Waltham Street from Crafts Street. MV2 had the right of way and MV1 failed to yield to MV2's right of way causing the collision to take place. Moran was advised of my investigation and issued Massachusetts Uniform Citation T0115914 via mail by request for Chapter 90, Section 8 (Fail To Yield To The Right Of Way At An Intersection.) Mr. Nadhir Tchikou operated his motor scooter at the time of the crash without a valid license/permit. Tchikou is also in violation of operation of a motor scooter after dark (prohibited). The crash occurred at 2147 hours. Tchikou will be issued Massachusetts Uniform Citation T0115915 in hand at Brigham and Women's Hospital for Chapter 90, Section 1E (Unlicensed Operation Of A Motor Scooter) and Chapter 90, Section 1E (Operation Of A Motor Scooter After Sunset - Hours Restriction.)

On Tuesday, November 3, 2020, while assigned to Traffic unit 525, I responded to the intersection of Langley Road and Union Street, Newton for a report of a motor vehicle crash with injury. The weather at the time of the crash was clear and sunny. The road surface was dry. Langley Road and Union Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Victor Vayntraub (S86665758). Vayntraub stated they were operating their Volkswagen Touareg (MA: 7HP137) North on Langley Road towards Beacon Street. Vayntraub stated as he passed through the Union Street intersection of Langley Road, MV2 exited Union Street traveling East across the intersection and he was unable to avoid the collision. MV1's front end crashed into the passenger side of MV2. I observed significant damage to the front end of MV1. Tody's towing responded and removed MV1 from the roadway. The operator of MV1 was evaluated by Newton Medics on scene and signed a patient refusal. I spoke with the operator of MV2, Jean Scribner (S17093056). Scribner stated she as operating her 2016 Ford Explorer (MA: 274XPY) East on Union Street. Scribner stated she came to a stop at the stop sign on Union Street at Langley Road. Scribner stated she looked to her left and right, saw no vehicles, and proceeded to enter the intersection to cross over Langley road and continue straight. Scribner stated once in the intersection a white vehicle came out of nowhere and crashed into the passenger side of her vehicle. I observed significant damage to the passenger side of MV2. Tody's towing responded and removed MV2 from the roadway. The operator of MV2 was evaluated by Newton Medics on scene and signed a patient refusal. After speaking with both operators, the operator of MV2 (Scribner) failed to yield to MV1's right of way when she attempted to cross Langley Road from Union Street. Scribner was issued Massachusetts Uniform Citation T1442404 in hand for Chapter 89, Section 8 (Fail to Yield to the Right of Way at an Intersection). As a result of the crash, the municpal street sign at the intersection of Langley Road at Union Street (E) was damaged. Photos were taken and submitted to the IT Bureau.

I spoke with the reporting party and operator of MV1, Sungmi Lee (S54407875). Ms. Lee stated she was operating her 2017 Hyundai Santa Fe (MA: 6KR971) Eastbound through the second floor of the Wegmans parking garage, a public right of way. The area she was traveling through is designated for one was travel with a vehicle ramp to her right that allows access for vehicle to go up or come down from the third floor. Ms. Lee stated MV2 (2018 Jeep Compass, CT: AR52330), traveled down the ramp from the third floor and did not stop at the stop sign at the entrance to the second floor. Ms. Lee stated the front end of MV2 crashed into the passenger side of her vehicle. I observed damage to the passenger side area of MV1. Ms. Lee stated her neck was sore from the crash but declined having EMS respond to evaluate her. Ms. Lee stated both vehicles pulled over to exchange information after the crash. Ms. Lee stated the operator of the other vehicle, Avery Adams, stated to her she was at fault for the crash. Ms. Lee stated she noticed the license Ms. Adams showed her was expired. Ms. Adams left the area after they exchanged information and Ms. Lee contacted Newton PD about Ms. Adams' license status. I queried Ms. Adams' license and registration status through Newton PD Dispatch who informed me both were suspended. I spoke with Ms. Adams via telephone. Ms. Adams stated she was traveling down the ramp to the second floor of the parking garage and came to stop at the stop sign. Ms. Adams stated she began making her turn onto the second floor when MV1 sped up to go around her. Ms. Adams stated the front driver side of her vehicle crashed into the rear passenger side of MV2. Ms. Adams was advised of her license and registration status and was mailed Massachusetts Uniform Citation T0115918 for Chapter 90, Section 23 (Operate MV with a Suspended License) and Chapter 90, Section 23 (Operate MV with a Suspended Registration).

On Thursday 1/7/21 at approx 1851hrs, while assigned to N494, I responded to Comm Ave and Centre St for a 2 car MVA. Operator of MV2 stated that he was attempting to take a left turn on to Centre St NB when MV1 came straight at him before both vehicles collided. He said the light was green and he was in the left turn only lane on Comm Ave EB. Operator of MV1 stated that he was driving WB on Comm Ave and while the signal was green tried to go straight across the intersection. He said that MV2 struck his vehicle in the middle of the intersection. He said he briefly lost control and his car turned onto to Centre St SB and stopped at the corner of Homer St. It should be noted that driver side airbags were deployed. Both occupants in MV1 signed refusals with medics. M1 was towed by Tody's Towing service. MV2 had an expired registration and a citation was issued to Dos Santos. Witness statement 1 Canzano stated that she was travelling WB on Comm Ave in the left turn only lane(ma reg 6TL410). She said that the light was green for all Comm Ave traffic and she stopped to yield to on coming traffic. Canzano stated that MV1 passed her on her right and attempting to cross the intersection when MV2, failing to yield, tried to turn left on to Centre St. She witnessed both cars collide. Witness statement 2 Danberg stated that he was traveling NB on Centre St and was coming to a stop at the Comm Ave intersection. He said that it appeared that all Comm Ave had a green light and that MV2 failed too yield to MV1. He states that his dash cam captured that accident and will email me the video.

Operator of MV1 (ma reg 8VW861) Stated she was heading EB on Boylston when she stopped at the intersection of Boylston St and Chestnut St. Operator of MV1 stated she observed a vehicle stopped at Chestnut and waited till the vehicle went throught the intersection before proceeding. Operator of MV1 continued EB on Boylston when her vehicle was struck on the front passenger side door by MV2. Operator of MV1 stated she was about 3/4 throught the intersection when the vehicle struck her car. Operator of MV2 ma reg (7HG793) stated she was driving NB on Chestnut Street when she stopped at the intersection of Boylston. Operator of MV2 thought it was safe to continue NB on Chestnut Street and did not observe MV1 coming from Boylston St. The passenger in MV1 stated while sitting in the front passenger seat he observed MV2 fail to stop at the stop sign and hit MV1. All three parties involved in the accident were all complaining about injuries. Medic 4, Medic 5 and Engine 7 arrived on scene to assist. All three parties were transported to NWH with non-fatal injuries. Tody's was also called to the scene to tow both vehicles. The owner of MV1 who was the passenger was cited (MA Citation 949189AA) 90/34J uninsured motor vehicle and 90/9/B unregistered motor vehicle. The owner of MV2 was cited (MA Citation 949218AA) 89/9 failure to stop at stop sign. Both citations were mailed out. All parties were informed of the citations being issued and mailed out. I have also added an incident report#21013647 due to the charges against the owner of MV1. All other information will be included in that report.

Bicyclist Cory Small stated that on 06/10/2021 at 17:48 hours he was riding his Specialized carbon frame bike in the westbound bike lane on Beacon St near Garland Rd. Beacon St and Garland Rd are both public ways in the City of Newton. Small stated that westbound Beacon St traffic was stopped at the intersection of Walnut St. Small stated that he continued riding westbound in the bike lane while traffic was stopped. Small stated that as he was crossing over the intersection of Garland Rd. when a vehicle that was driving in the eastbound lane of Beacon St turned left onto the northbound lane of Garland Rd and into his path of travel. Small stated that when the vehicle turned onto Garland Rd it caused him to crash into the rear passenger side of it. As a result of the crash Small had lacerations to his left hand and leg. Small's left ankle became swollen due to the crash. Small was transported via EMS to Newton Wellesley Hospital for treatment. Small was wearing a helmet at the time of the crash. Operator of vehicle one ( 2012 Volvo XC70 MA reg 2FJK50 ) Matthew Ristuccia stated that he was driving eastbound on Beacon St and was turning left onto the northbound lane of Garland Rd when the crash occurred. Ristuccia stated that he did not see Small prior to the crash. Ristuccia stated that he and his two sons who were passengers were not injured. Ristuccia's vehicle had minor scrape marks on the rear and center passenger side. I took photos of Ristuccia's vehicle and the disk was downloaded by the NPD's IT Bureau. Small's bike had damage to the left hand brake, and possible frame damage. Based upon statements made to me by Small and Ristuccia and my observation of the vehicle and bike, I issued Ristuccia MA Uniform Citation T1447316 and cited him for a violation of MGL 90/14 , failure to yield to an oncoming bicyclist when turning left. I explained to Ristuccia his payment and appeal options.

The operator of veh#2 admitted to hitting the gas instead of the brake. Operator of veh #2 cited in hand Mass. #T2080855 N.C.O.-Failure to use care in starting. I went inside Loyal Companion and spoke with Manager Micheal Wade. Wade stated no pets were hurt during the accident and he had already notified the buildindgs mainternece company to repsonde and board up the windows.

On Tuesday, June 22, 2021, while on patrol in Traffic unit N525, I observed a motor vehicle crash at the intersection of Washington Street and Armory Street, Newton. Washington Street and Armory Street are both public ways maintained by the City of Newton. The weather at the time of the crash was overcast with light rain. The road surface was wet. While parked in the Armory Street side parking lot (W) 1121 Washington Street, I observed a white 2016 Mazda CX-3 (MA: 1924DD) accelerate from it's stopped position on Armory Street and onto Washington Street to merge into traffic. I then observed the Mazda's front driver side bumper/fender/wheel area crash into the front passenger side bumper/fender/wheel area of a passing 2016 black Ford Fusion (MA: 3ZJ461) . The Ford was traveling on Washington Street (W) and had the right of way. The operator of the Mazda, Ms. Marcella Velasco (S28312041) was provided Massachusetts Uniform Citation 047509AB in hand for Chapter 89, Section 9 (Fail to Yield to the Right of Way). The Mazda was removed from the roadway by Tody's Towing. I provide transportation for Ms. Velasco to the Enterprise Rent-A-Car located at 1229 Washington Street, Newton due to her vehicle being disabled.

On Friday, July 16, 2021, while assigned to Traffic unit N525, I responded to the intersection of Commonwealth Avenue and Temple Street, Newton for a crash involving a City if Newton Fire Department vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. Commonwealth Avenue and Temple Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Mr. Jacob Forbes. Mr. Forbes stated he was at a stop sign on Temple Street and was attempting to cross Commonwealth Avenue (N) from Temple Street. Mr. Forbes stated he looked to his right, and then to his left and did not see any vehicle approaching. Mr. Forbes stated as the began to enter the intersection he looked to his right and saw a vehicle pulling over to the side of the road and a Newton Fire truck with it's lights on was heading towards him. Mr. Forbes stated he could not avoid the collision and the front end of MV2 crashed into his passenger side. Mr. Forbes reported no injuries and signed a patient refusal with Fallon Medics. I observed damage to the front passenger side area of MV1 and it was removed from the roadway by Tody's Towing. MV2 is a City of Newton Fire Department vehicle "C2". It is a 2019 Ford Expedition (MF455). The vehicle was responding to a Fire Alarm Activation call with emergency lights and siren activate at the time of the crash. The occupants of the vehicle were taken to Newton Wellesley Hospital for evaluation. I observed front end and driver side damage to MV2. There was air bag deployment inside the vehicle, on both the front driver and passenger side. Tody's Towing removed MV2 from the roadway. Photos were taken of the crash scene and submitted to the IT Bureau. I responded to Newton Wellesley Hospital and spoke with Newton Firefighter John Morris (Driver) and Deputy Chief Michael Mcnamara (passenger) in the emergency room. Both parties stated they were responding to a call with emergency lights and siren activated. FF Morris stated as he approached the intersection of Commonwealth Avenue (W) and Temple Street, he observed a black SUV exiting Temple Street onto Commonwealth Avenue. FF Morris stated as they were passing he believed MV1 tried to speed up to cross Commonwealth Avenue. MV2 then crashed into MV1. As a result of the crash, FF Morris suffered right hand/wrist injury. Both FF Morris and Deputy Chief Mcnamara stated they had slight respiratory issues from breathing in the powder from the air bag deployment. MV1 failed to yield to MV2's right of way as he attempted to cross Commonwealth Avenue from Temple Street. The operator of MV1, Mr. Jacob Forbes will be mailed Massachusetts Uniform Citation 071113AB for Chapter 89, Section 8 (Fail to Yield at Intersection).

On 8/10/2021 at approx 0851hrs while assigned to 497 I responded to the intersection of Washington St at Grove St for a report of a motor vehicle/bike crash with minor injuries. Upon arrival I met with Montgomery Curren a 15 year old adolescent male who related he was in the right travel lane w/b on Washington St crossing the intersection of Grove St when Ma Reg 3SN235 pulled in front of him taking a left from the e/b lane of Washington St onto Grove St. Curren was wearing a bicycle helmet, had minor scrapes and minor damage to his handlebars. I spoke with Triaca Fiorella who stated she was taking a left onto Grove St from Washington St, stopped to see if any cars were coming , when she saw no cars she took the left and Curren ran into the side of her car. Fiorella stated she believed Curren was "speeding" on his bicycle. Mass Uniform Citation T2015318 issued to Fiorella for violation MGL Ch 89 Sec 8 Fail to Yield Right of Way. Currens father , James Curren, responded to the scene to sign a patient refusal for Curren.

MV#1 AFTER THE IMPACT WAS FORCED UP ONTO THE SIDEWALK DAMAGING A CITY "NO PARKING" SIGN. OFFICER WILSON TOOK PICTURES OF THE CITY PROPERTY DAMAGE. AS THE OPERATOR OF MV#1 WAS BEING EVALUATED BY THE MEDICS I ASKED HIM IF HE HAD ANY INJURIES AND HE STATED HE DID NOT. I THEN INFORMED HIM THAT HE HAD A STOP SIGN AT THE INTERSECTION WHICH HE AGREED BUT STATED HE DID NOT SEE MV#2 WHILE PULLING OUT ONTO LANGLEY ROAD. I INFORMED THE OPERATOR OF MV#1 THAT BASED ON THE STATEMENTS FROM BOTH HIM AND THE OPERATOR OF MV#2, I MADE THE DETERMINATION THAT HE WAS AT FAULT, FAILED TO YIELD, AND THAT OPERATOR OF MV#2 HAD THE RIGHT OF WAY. I INFORMED THE OPERATOR OF MV#1 THAT I WOULD NOT BE ISSUING HIM A CITATION AT THIS TIME. THE OPERATOR OF MV#1 THEN BECAME ARGUMENTATIVE INSIDE THE AMBULANCE STATING TO ME "GET THE HELL AWAY FROM ME I AM DONE WITH YOU". THE OPERATOR OF MV#1 SIGNED A PATIENT REFUSAL AND MV#1 WAS TOWED BY TODY'S. MV#1 HAD MAJOR DAMAGE TO THE DRIVERS REAR, SIDE, AND FRONT, ALONG WITH SIDE AIR BAG DEPLOYMENT. MV#2 HAD DAMAGE TO THE P/S FRONT BUT WAS ABLE TO DRIVE AWAY. IT APPEARED THAT THERE COULD POSSIBLY BE UNDERLINING DAMAGE TO MV#2.

Witness, who was in the home at #22 Brae Burn Rd, describes MV#2 as JSC Transportation van that picked up a student at #22 Brae Burn Rd. Request Traffic Bureau follow up with JSC Transportation. Traffic Bureau update (Officer Gaudet): On November 23, 2021, I contacted JSC Transportation to follow up with them and get information for the operator of the van that was involved in the crash. JSC Transportation identified the operator of the vehicle as Ms. Brittany Flannery(S78277983) and provided me with her contact information. I also asked for information on the student that was on the vehicle at the time of the crash, Davine Archibald. JSC Transportation stated they did not realize this was the student on the van at the time of the crash. They immediately contact Davine's mother, Beula Effion to make her aware of the crash. I also contacted Ms. Effion to make her aware of the crash. Ms. Effion stated she received a call from JSC Transportation and that she is ok with everything. Ms. Effion stated she does not believe Davine was injured. Ms. Effion was advised. I spoke with Ms. Flannery about the crash. Ms. Flannery stated she was operating the JSC Transportation van (2019 Ford Transit, MA SPN: SP10940) involved in the crash. Ms. Flannery stated she was on her way to a pick up and was coming down Brae Burn Road. Ms. Flannery stated there were a couple of work vans parked on either side of the driveway where her pick up was. Ms. Flannery stated she pulled off the street and tried to park in between the two work vehicles on either side of the driveway. Ms. Flannery stated as she attempted to do this, the back passenger side of her van clipped the back passenger side of the van parked to her right. Ms. Flannery stated she asked the 1 passenger on the van, Davine Archibald, if she was ok. Ms. Flannery stated Davine stated she was. Ms. Flannery stated she exited the vehicle and checked the damage. Ms. Flannery reported the damage to the van as a scrape and scratch on the rear passenger side. Ms. Flannery stated she continued with her pick up at 22 Brae Burn Road and told the resident there she was going to contact the company that owns the van later. Ms. Flannery then left the area. Ms. Flannery stated she dropped the students off at the Newton Early Education Center on Jackson Road. I asked Ms. Flannery why she did not contact the Newton Police Department to report the crash and help her exchange information with the parked vehicle she hit. Ms. Flannery stated she did not know she had to because the vehicle was unoccupied. Ms. Flannery was advised that she needs to report being involved in a collision, especially when transporting children for JSC Transportation. Ms. Flannery will be cited with Massachusetts Uniform Citation 203510AB for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Brae Burn Road is a public way maintained by the City of Newton.

The operator of MV 2 (Mr. Warren Freedenfeld) was cited for MGL chapter 89/8 (fail to yeild to oncoming traffic). MV 2 sustained major damage to the front left wheel rendering it disabled. MV 2 was towed by a private tow (AAA). There were no injuries to either driver and they both signed patient refusals with Fallon medics.

MV 2 sustained minor damage to the front left. The operator of MV 2 was uninjured. The registration of MV 2 was expired at the time of the crash. The operator and owner of MV 2, Mr. Seton Murphy, was able to register the vehicle at the scene. Mr. Murphy recieved a citation for an expired registration. Neither vehicle required a tow.

On Sunday 6/12/22 at approximately 1333 hours while assigned to marked unit n498 I was dispatched to the area of 832 Dedham Street for a report of a 2 car motor vehicle crash with no injuries. Upon arrival I spoke with the operator, GERSHON Andrew, of motor vehicle #1 (6HS292). GERSHON states he was traveling Westbound on Dedham Street toward Brookline Street when he approached the driveway at 832 Dedham Street and motor vehicle #2 started to pull out in front of him. GERSHON states as he saw the vehicle pull out he swerved into oncoming traffic to avoid the collision, however motor vehicle #2 struck the passenger side of his vehicle. After the collision GERSHON attempted to pull back into his lane of travel, but lost control causing him to overcorrect and go off the road on the westbound side of the road in front of 824 Dedham Street causing some damage to the front landscaping of the yard. Secondly, I spoke with the operator, JIN Zhien, of motor vehicle #2 (EV87SA). JIN states she was attempting to pull out of the driveway of 832 Dedham Street and didn't see motor vehicle #1. JIN states she was distracted because there was a lot going on inside the car. She stated she was not on her cell phone. Finally I also spoke with a witness, CURTIS Ron, who was travelling in the opposite direction and witnessed the crash. Ron stated he saw motor vehicle #1 travelling straight in the opposite direction on Dedham Street. He saw the Tesla (MV#2) attempting to pull out of the driveway at 832 Dedham Street and drive into the side of MV#1. Ron stated he also observed MV#1 swerve into his lane of traffic trying to avoid the collision and then spinning out into the front of 824 Dedham Street. Taking the witness, operator, and passenger statements into account as well as my observations, I determined the operator, (JIN Zhien) of MV#2 (EV87SA) is at fault for the crash and she has been issued Massachusetts Uniform Citation T2012918 C89 S8 Failure to Yield Right of Way.
